Hedging to save face: a linguistic analysis of written comments on in-training evaluation reports.

Written comments on residents' evaluations can be useful, yet the literature suggests that the language used by assessors is often vague and indirect. The branch of linguistics called pragmatics argues that much of our day to day language is not meant to be interpreted literally. Within pragmatics, the theory of 'politeness' suggests that non-literal language and other strategies are employed in order to 'save face'. We conducted a rigorous, in-depth analysis of a set of written in-training evaluation report (ITER) comments using Brown and Levinson's influential theory of 'politeness' to shed light on the phenomenon of vague language use in assessment. We coded text from 637 comment boxes from first year residents in internal medicine at one institution according to politeness theory. Non-literal language use was common and 'hedging', a key politeness strategy, was pervasive in comments about both high and low rated residents, suggesting that faculty may be working to 'save face' for themselves and their residents. Hedging and other politeness strategies are considered essential to smooth social functioning; their prevalence in our ITERs may reflect the difficult social context in which written assessments occur. This research raises questions regarding the 'optimal' construction of written comments by faculty.

The aim of this study was to better understand the utility of using the Parents' Evaluation of Developmental Status (PEDS) in well-child visits by analyzing themes and patterns in parents' written responses on the PEDS form. We reviewed a consecutive sample of medical records with PEDS forms for children aged 6 months to 9 years (site 1) and 3 to 5 years (site 2). We recorded the concerns that parents identified in response to the 10 PEDS questions along with demographic information. We then categorized parents' written comments about those concerns according to comment content. We used qualitative and quantitative methods for analysis. We collected 752 PEDS forms. Ninety percent of the parents endorsed at least 1 concern (94.6% on the English forms versus 69.7% on the Spanish forms; P Parents qualified 27.5% of their concerns with a written comment. In 23.9% of cases in which parents identified a concern and provided a written comment, the content of the comment did not match the question's intent; rates of mismatch were similar for the English and Spanish forms. Among comments regarding behavioral concerns, 12% reflected a misunderstanding of age-appropriate behavior. Medical concerns accounted for 14.1% of the comments; these concerns were more common on English forms (61.3%) than on Spanish forms (1.7%) (P Parents frequently used the PEDS forms to communicate additional concerns regarding their child or provide positive feedback on their child's progress. The inappropriate developmental expectations, limited health literacy, and culturally distinct comments on the PEDS forms reinforce the importance of using screening tools to enhance the care provided during visits but not to replace patient-provider communication.

Comments on residents' in-training evaluation reports (ITERs) may be more useful than scores in identifying trainees in difficulty. However, little is known about the nature of comments written by internal medicine faculty on residents' ITERs. Comments on 1,770 ITERs (from 180 residents in postgraduate years 1-3) were analyzed using constructivist grounded theory beginning with an existing framework. Ninety-three percent of ITERs contained comments, which were frequently easy to map onto traditional competencies, such as knowledge base (n = 1,075 comments) to the CanMEDs Medical Expert role. Many comments, however, could be linked to several overlapping competencies. Also common were comments completely unrelated to competencies, for instance, the resident's impact on staff (813), or personality issues (450). Residents' "trajectory" was a major theme (performance in relation to expected norms [494], improvement seen [286], or future predictions [286]). Faculty's assessments of residents are underpinned by factors related and unrelated to traditional competencies. Future evaluations should attempt to capture these holistic, integrated impressions.

Full Text Available Abstract Background There is increasing interest by chiropractors in North America regarding integration into mainstream healthcare; however, there is limited information about attitudes towards the profession among conventional healthcare providers, including orthopaedic surgeons. Methods We administered a 43-item cross-sectional survey to 1000 Canadian and American orthopaedic surgeons that inquired about demographic variables and their attitudes towards chiropractic. Our survey included an option for respondants to include written comments, and our present analysis is restricted to these comments. Two reviewers, independantly and in duplicate, coded all written comments using thematic analysis. Results 487 surgeons completed the survey (response rate 49%, and 174 provided written comments. Our analysis revealed 8 themes and 24 sub-themes represented in surgeons' comments. Reported themes were: variability amongst chiropractors (n = 55; concerns with chiropractic treatment (n = 54; areas where chiropractic is perceived as effective (n = 43; unethical behavior (n = 43; patient interaction (n = 36; the scientific basis of chiropractic (n = 26; personal experiences with chiropractic (n = 21; and chiropractic training (n = 18. Common sub-themes endorsed by surgeon's were diversity within the chiropractic profession as a barrier to increased interprofessional collaboration, endorsement for chiropractic treatment of musculoskeletal complaints, criticism for treatment of non-musculoskeletal complaints, and concern over whether chiropractic care was evidence-based. Conclusions Our analysis identified a number of issues that will have to be considered by the chiropractic profession as part of its efforts to further integrate chiropractic into mainstream healthcare.

The Department of Energy (DOE) issued a Record of Decision for the Storage and Disposition of Weapons-Usable Fissile Materials Programmatic Environmental Impact Statement (S ampersand D PEIS) (DOE/EIS-0229) on January 14, 1997. In that Record of Decision, DOE stated its decision to pursue a strategy for plutonium disposition that allows for immobilization of surplus weapons plutonium in glass or ceramic forms and irradiating the surplus plutonium as mixed oxide (MOX) fuel in existing reactors, while reserving the option to immobilize all the surplus weapons plutonium. The Department also decided that the extent to which either or both of these disposition approaches would ultimately be deployed would depend in part upon future National Environmental Policy Act (NEPA) review for surplus weapons plutonium disposition. On May 22, 1997, DOE published in the Federal Register (62 FR 28013) a Notice of Intent to prepare an environmental impact statement (tiered from the S ampersand D PEIS) on the disposition of United States' surplus weapons-usable plutonium. The purpose of the Notice of Intent was to describe DOE's proposed action, to solicit public input, and to announce the schedule for the public scoping meetings. During the public scoping period (May 22 - July 22, 1997), the public was invited to submit written comments by U.S. mail, fax, or through the Office of Fissile Materials Disposition's Website, as well as to provide oral comments by voicemail or by participating in public scoping meetings. An independent technical review (peer review) was conducted during the period of September 15--17, 1992. The review was conducted by C. Warren Ankerberg (Geraghty and Miller, Inc., Tampa, Florida) and Don Messinger (Roy F. Weston, Inc., West Chester, Pennsylvania). The review was held at Jacobs Engineering in Albuquerque, New Mexico, and at the Shiprock, New Mexico, site. The peer review included a review of written documentation [water sampling standard operating procedures (SOP)], an inspection of technical reports and other deliverables, a review of staff qualifications and training, and a field visit to evaluate the compliance of field procedures with SOPS. Upon completion of the peer review, each reviewer independently prepared a report of findings from the review. The reports listed findings and recommended actions. This document responds to the observations, comments, and recommendations submitted by Don Messinger following his review. The format of this document is to present the findings and recommendations verbatim from Mr. Messinger's report, followed by responses from the UMTRA Project staff. The Quality of Written Feedback by Attendings of Internal Medicine Residents.

Attending evaluations are commonly used to evaluate residents. Evaluate the quality of written feedback of internal medicine residents. Retrospective. Internal medicine residents and faculty at the Medical College of Wisconsin from 2004 to 2012. From monthly evaluations of residents by attendings, a randomly selected sample of 500 written comments by attendings were qualitatively coded and rated as high-, moderate-, or low-quality feedback by two independent coders with good inter-rater reliability (kappa: 0.94). Small group exercises with residents and attendings also coded the utterances as high, moderate, or low quality and developed criteria for this categorization. In-service examination scores were correlated with written feedback. There were 228 internal medicine residents who had 6,603 evaluations by 334 attendings. Among 500 randomly selected written comments, there were 2,056 unique utterances: 29% were coded as nonspecific statements, 20% were comments about resident personality, 16% about patient care, 14% interpersonal communication, 7% medical knowledge, 6% professionalism, and 4% each on practice-based learning and systems-based practice. Based on criteria developed by group exercises, the majority of written comments were rated as moderate quality (65%); 22% were rated as high quality and 13% as low quality. Attendings who provided high-quality feedback rated residents significantly lower in all six of the Accreditation Council for Graduate Medical Education (ACGME) competencies (p service examination scores. Most attending written evaluation was of moderate or low quality. Attendings who provided high-quality feedback appeared to be more discriminating, providing significantly lower ratings of residents in all six ACGME core competencies, and across a greater range. Attendings' negative written comments on medical knowledge correlated with lower in-service training scores.

There remains debate regarding the value of the written comments that medical students are traditionally asked to provide to evaluate the teaching they receive. The purpose of this study was to examine written teaching evaluations to understand how medical students conceptualize teachers' behaviors and performance. All written comments collected from medical students about teachers in the two surgery clerkships at the University of Alberta in 2009-2010 and 2010-2011 were collated and anonymized. A grounded theory approach was used for analysis, with iterative reading and open coding to identify recurring themes. A framework capturing variations observed in the data was generated until data saturation was achieved. Domains and subdomains were named using an in situ coding approach. The conceptual framework contained three main domains: "Physician as Teacher," "Physician as Person," and "Physician as Physician." Under "Physician as Teacher," students commented on specific acts of teaching and subjective perceptions of an educator's teaching values. Under the "Physician as Physician" domain, students commented on elements of their educator's physicianship, including communication and collaborative skills, medical expertise, professionalism, and role modeling. Under "Physician as Person," students commented on how both positive and negative personality traits impacted their learning. This framework describes how medical students perceive their teachers and how they use written language to attach meaning to the behaviors they observe. Such a framework can be used to help students provide more constructive feedback to teachers and to assist in faculty development efforts aimed at improving teaching performance.

On November 22, 1989, the Environmental Protection Agency (EPA, the Agency) proposed a rule to prohibit the land disposal of the final third of the schedule of restricted hazardous wastes, known as the Third Third (see 54 Federal Register 48372). The proposal outlined treatment standards and effective dates of prohibition for these wastes as well as for some First and Second Third soft hammer wastes, characteristic wastes, certain waste groups (e.g., multi-source leachate, radioactive mixed waste), and newly listed wastes. The proposed rule was issued pursuant to the 1984 Hazardous and Solid Waste Amendments (HSWA) to the Resource Conservation and Recovery Act (RCRA). The Agency requested public comment on the proposal. Full Text Available Launching the distance-learning student-tutor interaction process, tutors of the first module of the M.Ed in English course at the HOU lay the foundations of academic student autonomy by means of providing – inter alia -- the appropriate written feedback on written assignments. In doing so, they need to gauge the content and form of their written comments systematically with regard to both output- and student-, that is human factor-related issues (cf. Goldstein, 2004, the latter being particularly relevant to the distance-learning context. In this article we discuss tutor policy as well as tutor perceptions (cf. Lee, 2004, 2009 among others regarding written feedback on students’ academic assignments in terms of aspects of deviance treated and the relative gravity of ‘global’ and ‘local’ errors (e.g. Ferris, 2002, the directness of the correction, the punitive or facilitative nature of the comments provided as well as the relative balance of student strengths and weaknesses on the tutor’s comment agenda (cf. Hyland & Hyland, 2006. The role of the tutor as an assessor and/or counsellor is explored and the importance of striking a delicate balance between the two, especially in a context where face-to-face feedback opportunities are severely restricted, is underscored. We suggest that distance-learning feedback practices may need to be at least partially individualized to maximize student response and meet the goal of ‘informed autonomy’.

During the 60-day period provided for comments on the ''Summary of the Results of the Evaluation of the WIPP Site and Preliminary Design Validation Program'' (WIPP-DOE-161), written submittals and hearing testimony from about 133 individuals, 7 citizens groups and 6 state agencies were received by the Department of Energy (DOE). Approximately 25% of the public comment submittals were positive statements supporting the WIPP, with the remaining 75% reflecting concern with one or more aspects of the project. A portion of the state's comment package (submitted by the Governor of New Mexico) contained concerns relevant to WIPP which were unrelated to site suitability. In April 1990 the US Department of Energy (DOE) issued a site- specific Five-Year Plan (DOE-RL 1989a) on Hanford's cleanup for public review and comment. The plan guides Hanford in carrying out DOE's national plan for environmental restoration and waste management. During the 90-day public comment period, DOE held nine public meetings to answer questions and gather comments on the plan. This report is in three main sections. The first presents consolidated public comments and responses. These were compiled from both verbal comments from public meetings and written comments. The second section contains comments not responded to in this plan. Those comments were outside this plan's scope, related to how we gathered public comments, or intended for and directed to DOE-Headquarters. In the appendixes are the written comment letters we received and a short glossary and list of special terms we use. The source of comments is shown in parentheses after the comment. The individual who made the comment, the city of the public meeting, or the organization name is generally used. When several sources gave the same comment, a source was not listed. 26 refs

Although scores on in-training evaluation reports (ITERs) are often criticized for poor reliability and validity, ITER comments may yield valuable information. The authors assessed across-rotation reliability of ITER scores in one internal medicine program, ability of ITER scores and comments to predict postgraduate year three (PGY3) performance, and reliability and incremental predictive validity of attendings' analysis of written comments. Numeric and narrative data from the first two years of ITERs for one cohort of residents at the University of Toronto Faculty of Medicine (2009-2011) were assessed for reliability and predictive validity of third-year performance. Twenty-four faculty attendings rank-ordered comments (without scores) such that each resident was ranked by three faculty. Mean ITER scores and comment rankings were submitted to regression analyses; dependent variables were PGY3 ITER scores and program directors' rankings. Reliabilities of ITER scores across nine rotations for 63 residents were 0.53 for both postgraduate year one (PGY1) and postgraduate year two (PGY2). Interrater reliabilities across three attendings' rankings were 0.83 for PGY1 and 0.79 for PGY2. There were strong correlations between ITER scores and comments within each year (0.72 and 0.70). Regressions revealed that PGY1 and PGY2 ITER scores collectively explained 25% of variance in PGY3 scores and 46% of variance in PGY3 rankings. Comment rankings did not improve predictions. ITER scores across multiple rotations showed decent reliability and predictive validity. Comment ranks did not add to the predictive ability, but correlation analyses suggest that trainee performance can be measured through these comments.

Griebeler and Werner offer a formal comment on Myhrvold, 2016 defending the conclusions of Werner and Griebeler, 2014. Although the comment criticizes several aspects of methodology in Myhrvold, 2016, all three papers concur on a key conclusion: the metabolism of extant endotherms and ectotherms cannot be reliably classified using growth-rate allometry, because the growth rates of extant endotherms and ectotherms overlap. A key point of disagreement is that the 2014 paper concluded that despite this general case, one can nevertheless classify dinosaurs as ectotherms from their growth rate allometry. The 2014 conclusion is based on two factors: the assertion (made without any supporting arguments) that the comparison with dinosaurs must be restricted only to extant sauropsids, ignoring other vertebrate groups, and that extant sauropsid endotherm and ectotherm growth rates in a data set studied in the 2014 work do not overlap. The Griebeler and Werner formal comment presents their first arguments in support of the restriction proposition. In this response I show that this restriction is unsupported by established principles of phylogenetic comparison. In addition, I show that the data set studied in their 2014 work does show overlap, and that this is visible in one of its figures. I explain how either point effectively invalidates the conclusion of their 2014 paper. I also address the other methodological criticisms of Myhrvold 2016, and find them unsupported.
