    Full Text Available Racional - O megacólon tóxico é complicação grave e pouco freqüente na retocolite ulcerativa idiopática. Ocorre, geralmente, em pacientes cuja doença ativa é resistente ao tratamento clínico e, em algumas situações, o fator desencadeante é desconhecido. A infecção pelo cytomelogalovirus em seres humanos em geral é evento subclínico; entretanto, em pacientes imunocomprometidos a primoinfecção ou mesmo a reativação de infecção latente pode ter amplas repercussões clínicas, uma das quais o desencadeamento de megacólon tóxico. Objetivo - Descrever um caso de megacólon tóxico fatal por cytomegalovirose em paciente com retocolite ulcerativa idiopática. Paciente - Masculino, 38 anos, com clínica de diarréia e perda ponderal cuja investigação mostrou tratar-se de retocolite ulcerativa idiopática e para a qual foi instituída terapêutica imunossupressora vigorosa. Resultados - Evoluiu com quadro clínico de megacólon tóxico, sendo submetido a colectomia total. Complicações clínicas culminaram no óbito pós-operatório e o estudo anatomopatológico do cólon revelou doença ativa, associada a ulcerações confluentes na base das quais foram observadas células com alterações características da cytomegalovirose. Conclusão - A cytomegalovirose deve ser considerada como um dos agentes causadores de megacólon tóxico em retocolite ulcerativa.Background - The toxic megacolon is a rare and severe complication of ulcerative colitis. In general it complicates patients with active colitis that are resistant to clinical treatment and, in some cases, the developing factor is unknown. Cytomegalovirus infection in humans in general is a subclinical condition.     Full Text Available Racional - A doença de Crohn e a retocolite ulcerativa idiopática são consideradas pouco freqüentes nos países em desenvolvimento, sendo escassos os estudos sobre a sua ocorrência no Brasil. Objetivos - Estudar a freqüência de admissão de casos da doença de Crohn e da retocolite ulcerativa inespecífica em um hospital universitário ao longo de 20 anos (1980-99 e descrever características demográficas e clínicas desses casos. Métodos - Calculou-se a freqüência de admissão de casos da doença de Crohn e da retocolite ulcerativa inespecífica de janeiro de 1980 a dezembro de 1999 e analisaram-se todos os casos destas doenças admitidos nos últimos 10 anos desse período. Resultados - No período estudado, registraram-se 257 casos novos, sendo 126 da doença de Crohn e 131 da retocolite ulcerativa inespecífica. A freqüência de admissão de casos de ambas as doenças aumentou de 40 para 61 casos/10.000 atendimentos, do primeiro para o segundo qüinqüênio, com menor crescimento subseqüente, sendo que a doença de Crohn tornou-se, gradualmente, mais freqüente que a retocolite ulcerativa inespecífica. Em ambas as doenças, houve predomínio de casos do gênero feminino, na faixa etária entre 20 e 50 anos, da cor branca, do estado civil casado e de não-tabagistas. Ambas as doenças apresentaram-se com os sintomas digestivos próprios e não houve diferenças entre elas quanto às freqüências de manifestações sistêmicas e extra-intestinais (29,5% vs 23,3%, incluindo as tromboembólicas (5,9% vs 5,5%. Na doença de Crohn, 59,2% dos casos apresentaram complicações (obstrução e/ou perfuração, enquanto que 53,7% dos casos de retocolite ulcerativa inespecífica foram de formas mais graves. Nos casos de doença de Crohn com obstrução, o tabagismo foi significativamente mais freqüente que nas formas não-complicadas.     Full Text Available INTRODUÇÃO: Existe uma grande prevalência de manifestações extra-intestinais(MEI em portadores de doença de Crohn(DC e de retocolite ulcerativa(RCU, variando de 24 a 65%. OBJETIVO: Determinar a prevalência de MEI em RCU e DC, correlacionando com diagnóstico do tipo de doença inflamatória intestinal, extensão, tempo de evolução e atividade da doença. MÉTODOS: Mil pacientes foram avaliados no Hospital das Clínicas da FMUSP no período de 1984 a 2004. Foram estudadas manifestações articulares, dermatológicas, oftalmológicas, urológicas, hepáticas, pulmonares e vasculares. RESULTADOS: Foram estudados 468 pacientes com DC(46,8% e 532 com RCU(53,2%. Encontrados 627 pacientes (59,2% com RCU e 66,7% com DC com pelo menos uma forma de MEI. A média de tempo de doença dos pacientes com MEI foi de 10 anos. As MEI foram mais freqüentes após o início dos sintomas intestinais. CONCLUSÕES: Tanto na RCU quanto na DC,quanto maior a extensão da doença no cólon, maior a incidência de MEI. As manifestações urológicas foram mais freqüentes na DC. As manifestações articulares e dermatológicas foram mais prevalentes no sexo feminino nos dois grupos. Manifestações hepáticas foram mais prevalentes na DC. As manifestações articulares, dermatológicas e vasculares tiveram correlação com a atividade da doença intestinal em ambos os grupos.INTRODUCTION: There is great prevalence of extra-intestinal manifestations (EIM in Crohn's disease(CD and ulcerative rectocolitis (UR, varying from 24 to 65%.     Full Text Available pyoderma gangrenosum is a rare inflammatory skin condition characterized by progressive and recurrent skin ulceration of destructive course. It is usually associated with rheumatoid arthritis, paraproteinemia, myeloproliferative diseases and inflammatory bowel diseases, especially non-specific ulcerative proctocolitis. In these situations, skin lesions are described as concurrent with the intestinal condition. However, reports on pyoderma gangrenosum preceding intestinal findings are less frequent. The authors describe a case of a woman with febrile condition associated with skin lesions diagnosed by biopsy as pyoderma gangrenosum. Two weeks later, she developed diarrhea, arthralgia and sepsis, being diagnosed as ulcerative proctocolitis. After the administration of the treatment for ulcerative proctocolitis, she showed improvements in sepsis care, remission of diarrhea and regression of skin lesions. This case highlights the importance of considering pyoderma gangrenosum as a manifestation associated with inflammatory bowel disease, regardless of its timing in relation to intestinal symptoms.Pioderma gangrenoso é uma forma de inflamação cutânea, caracterizada por ulceração progressiva e recorrente da pele, com curso destrutivo. Geralmente é associada à artrite reumatoide, paraproteinemia, doenças mieloproliferativas e doença inflamatória intestinal, em especial retocolite ulcerativa inespecífica. Em tais casos, as lesões cutâneas são descritas concomitantes ao quadro intestinal, porém, relatos com descrição de pioderma gangrenoso precedendo achados intestinais são menos frequentes. Os autores relatam caso de mulher com quadro febril associado a lesões cutâneas diagnosticadas por biópsia como pioderma gangrenoso.     Orthorhombic polycrystalline RCu 2 (R=Tb, Dy, Ho and Er) compounds were synthesized and the magnetic properties and magnetocaloric effect (MCE) were investigated in detail. All of the RCu 2 compounds are antiferromagnetic (AFM) ordered. As temperature increases, RCu 2 compounds undergo an AFM to AFM transition at T t and an AFM to paramagnetic (PM) transition at T N . Besides of the normal MCE around T N , large inverse MCE around T t was found in TbCu 2 compound. Under a field change of 0–7 T, the maximal value of inverse MCE is even larger than the value of normal MCE around T N for TbCu 2 compound. Considering of the normal and inverse MCE, TbCu 2 shows the largest refrigerant capacity among the RCu 2 (R=Tb, Dy, Ho and Er) compounds indicating its potential applications in low temperature multistage refrigeration. - Highlights: • Large inverse magnetocaloric effect is observed in TbCu 2 compound. • The AFM to AFM transition is observed in RCu 2 (R=Tb, Dy, Ho, Er) compounds. • The MCE performance of TbCu 2 compound is evaluated in a more comprehensively way.

    Highlights: ► Topotactic reduction of hexagonal RCu 0.5 Ti 0.5 O 3 phases is performed. ► TGA and magnetism indicate a formula of RCu 0.5 Ti 0.5 O 2.78 for the reduced phase. ► Topotactic reoxidation occurs on heating these phases to 400 °C in air. - Abstract: Hexagonal AMO 2 and AMO 3 phases have the same basic structure, and intermediate compositions for this structure have been prepared by topotactic oxidation of AMO 2 phases such as RCuO 2 , where R is a trivalent rare earth cation. We now find that such intermediate phases can also be prepared by topotactic reduction of hexagonal RCu 0.5 Ti 0.5 O 3 (R = Y, Tb-Lu) phases.     Full Text Available OBJETIVO: Identificar os sintomas depressivos mais freqüentes entre portadores da doença Retocolite Ulcerativa Idiopática (RCUI e compará-los a um grupo de controle. MÉTODO: A amostra compôs-se de 100 indivíduos adultos portadores de RCUI que freqüentavam o Ambulatório de Doenças Inflamatórias do Cólon do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo que foram comparados a 100 indivíduos isentos da doença. Após os procedimentos éticos devidos todos responderam ao Inventário de Depressão de Beck. RESULTADOS: Dos 21 sintomas depressivos, seis mostraram valores estatísticos significativos para: distorção da imagem corporal, inibição para o trabalho, fadiga, perda do apetite, preocupação somática e diminuição de libido. CONCLUSÃO: Verificou-se que os doentes apresentaram com mais freqüência sintomas depressivos da esfera somática quando comparados ao grupo de controle. A similaridade dos sintomas entre a doença e os sintomas depressivos faz com que os doentes sejam freqüentemente subdiagnosticados.OBJETIVO: Identificar los síntomas depresivos más frecuentes entre portadores de la enfermedad Rectocolitis Ulcerativa Idiopática (RCUI y compararla a un grupo de control.     A series of ternary compounds RCu 2 Si 2 (R: rare earth) has been grown in single-crystalline form by the flux method. The magnetic and electronic properties of these compounds were investigated by measuring the electrical receptivity, specific heat, magnetization, thermal expansion, ultrasonic and dHvA measurements. The anisotropy in susceptibility and magnetization is relatively small in RCu 2 Si 2 , reflecting the small splitting energy of the 4f electronic state by the crystalline electric field. The quadrupolar interaction most likely plays an important role in the relatively high antiferromagnetic ordering temperature in PrCu 2 Si 2 . Characteristic features of the observed Fermi surfaces in YbCu 2 Si 2 and YbCu 2 Ge 2 are discussed on the basis of the band calculations. (author)

    Full Text Available Dos pacientes con colitis ulcerativa fueron tratados con prednisona en una dosis de 30 a 5 mlgr. diarios durante un período de tiempo de cuarenta y cinco días y cuatro meses, respectivamente. Ambos pacientes tenían rayos X de estómago y duodeno negativos, así como cifras de uropepsina normal antes del tratamiento. Ambos presentaron durante el curso del tratamiento episodios epigástricos agudos, con signos radiológicos de gastritis y aumento apreciable en las cifras de uropepsina. Igualmente mejoraron al instalarse un régimen antiulceroso. Un paciente obtuvo cicatrización completa de las ulceraciones y el otro una mejoría apreciable.

    Full Text Available RACIONAL: A adesão ao tratamento medicamentoso nas doenças inflamatórias intestinais apresenta grande importância clínica e social. Porém, são escassos os estudos sobre este tema em nosso meio. OBJETIVO: Investigou-se a adesão ao tratamento medicamentoso prescrito, bem como a influência de alguns fatores sobre a adesão, de pacientes com doenças inflamatórias intestinais em acompanhamento em ambulatório de Gastroenterologia de um hospital universitário ligado ao Sistema Único de Saúde (SUS. MÉTODO: Realizou-se estudo transversal, com métodos indiretos, para avaliar a adesão ao tratamento de 26 casos da doença de Crohn, 26 pacientes com retocolite ulcerativa e 4 com colite indeterminada, que faziam uso contínuo de medicamentos, dos quais 89,3% eram fornecidos pelo SUS. Os pacientes foram classificados como tendo alto ou baixo grau de adesão, com base em dois diferentes instrumentos. RESULTADOS: A análise dos medicamentos utilizados revelou baixa adesão em 15,4% de pacientes com doença de Crohn e 13,3% com retocolite ulcerativa. Porém, o teste de Morisky, que avalia hábitos de uso dos medicamentos, mostrou 50% de baixa adesão na doença de Crohn e 63,3% na retocolite ulcerativa. Análise univariada evidenciou na doença de Crohn relação entre baixa adesão e maior duração da doença, estado marital instável, residência próxima ao hospital e envolvimento do cólon. Na retocolite ulcerativa observou-se relação entre baixa adesão e atividade da doença e maior número de medicamentos em uso. Porém, a análise multivariada não evidenciou relação estatisticamente significativa que indicasse influência de qualquer fator sobre a adesão ao tratamento. CONCLUSÕES: Proporções elevadas de pacientes com doenças inflamatórias intestinais apresentam hábitos de uso de medicamentos indicativos de baixa adesão, difíceis de prever a partir de dados demográficos e clínicos, o que aponta para a necessidade de maior

    Full Text Available Neste estudo se investigou as formas de enfrentamento do estresse e a presença dos sintomas depressivos em 100 indivíduos portadores de retocolite ulcerativa idiopática (RCUI, usuários do Ambulatório de Doenças Inflamatórias do HCFMUSP, que foram comparados com 100 indivíduos acompanhantes de pacientes de outra unidade da mesma instituição, isentos da doença. A metodologia consistiu de aplicar aos dois grupos o Inventário de Estratégias de Enfrentamento de Folkman e Lazarus e o Inventário de Depressão de Beck. As estratégias de enfrentamento mais utilizadas pelo grupo de doentes foram Suporte Social (47% e Reavaliação Positiva (40%, e no grupo controle ocorreu predomínio da estratégia Reavaliação Positiva (52%. Setenta e três pacientes, com média de idade de 34,6 (13-63 anos e com predomínio do sexo feminino (42 pacientes, 56,7% se submeteram ao procedimento para tratamento de RCU (46 pacientes - 63,0% e PAF(27 - 37,0%. Foram utilizadas as seguintes variantes técnicas: em S, de grande tamanho e ramo eferente longo (oito; em S pequeno e ramo eferente reduzido (22; em "dupla câmara" (20; em J (23. Todos os procedimentos foram seguidos da construção de ileostomia de proteção. De 1993 em diante, todos os pacientes tiveram a arcada do colo direito preservada. Setenta pacientes têm pelo menos um ano de pós-operatório e 61 têm dois anos ou mais com média de 7,01 (1-16 anos. Foram consideradas complicações precoces aquelas que ocorreram até o 30º dia de pós- operatório e tardias, após esse tempo. Resultados funcionais foram analisados após um ano do fechamento da ileostomia. Ocorreram 35 complicações precoces em 22 pacientes e 39 complicações tardias em 35 pacientes. Vinte e cinco pacientes não apresentaram complicações. As principais complicações foram: obstrução intestinal (19,1 %, fistulizações cutâneas, com vagina ou trato urinário (10,9%, isquemia de reservatório (parcial ou total, (9,5%, e ileíte do reservatório (pouchitis (6,8%. Nove pacientes (12,3% têm ileostomia funcionante, sendo que sete pacientes têm ainda o reservatório mantido no lugar e dois tiveram-no ressecado. A mortalidade diretamente relacionada ao procedimento foi em dois pacientes, mas outros quatro pacientes evoluíram tardiamente ao óbito, por causas como desnutrição crônica e tumor de cerebelo.   16. Coercivity enhancement in hot deformed Nd2Fe14B-type magnets by doping low-melting RCu alloys (R = Nd, Dy, Nd + Dy)

    Magnetic properties of the anisotropic NdFeB magnets prepared by hot pressing followed by die-upsetting NdFeB MQU-F powders doped with low-melting RCu alloy powders were explored, where RCu stands for Nd70Cu30, Dy70Cu30 and (Nd0.5Dy0.5)70Cu30, respectively. In addition, the post-annealing at 600 °C was employed to modify the microstructures and the magnetic properties of the hot deformed magnets. It is found that doping RCu alloy powders is effective in enhancing the coercivity of the hot deformed NdFeB magnets from 15.1 kOe to 16.3-19.5 kOe. For Nd70Cu30-doped magnets, the increment of coercivity is only 1.2 kOe. Meanwhile, Dy70Cu30-doped and (Nd0.5Dy0.5)70Cu30-doped magnets show an almost identical enhancement of coercivity of about 4.4 kOe. Importantly, the latter magnet shows a beneficial effect of reducing the usage of Dy from 1.6 wt% to 0.8 wt%. TEM analysis shows that nonmagnetic Nd, Dy and Cu appear at grain boundary and isolate the magnetic grains, leading to an enhancement of coercivity. Doping lower melting point Dy-lean (Nd0.5Dy0.5)70Cu30 powders into commercial MQU-F powders for making high coercivity hot deformed NdFeB magnets might be a potential and economic way for mass production.

    Single crystals of tetragonal RCuGa{sub 3} (R=La, Pr, Nd and Gd), with BaNiSn{sub 3} type structure (space group I4 mm), have been grown by high temperature solution growth technique using Ga as flux. Their magnetic properties were determined by heat capacity and the measurement of magnetization and electrical resistivity along [100] and [001] directions. Except LaCuGa{sub 3}, the compounds order magnetically. PrCuGa{sub 3} undergoes a ferromagnetic transition with Curie temperature of 4.6 K. NdCuGa{sub 3} shows a bulk magnetic transition at 3.3 K. The data on GdCuGa{sub 3} indicate combined characteristics of spin glass and antiferromagnetic behavior at low temperatures. From the Schottky heat capacity data the crystal electric field level energy spectra have been determined. Further we have performed temperature dependent measurements of electron spin resonance (ESR) on GdCuGa{sub 3} between 11 K and room temperature. The ESR data indicate an enhancement of magnetic fluctuations associated with spin reorientation and both homogeneous and inhomogeneous thermal broadening of the linewidth. - Highlights: • RCuGa{sub 3} (R=La, Pr, Nd and Gd) single crystals were grown by flux technique. • The compounds exhibit long range magnetic ordering. • Crystal electric field levels have been derived from Schottky heat capacity. • Electron spin resonance (ESR) studies of GdCuGa{sub 3} are also discussed.

    We report the effect of replacing Cu by Pd in RCu 5 (R=Pr, Nd, Sm and Eu). The parent RCu 5 compounds crystallize in the hexagonal CaCu 5 -type structure. The hexagonal symmetry is retained in PrCu 4 Pd and EuCu 5-x Pd x (x=1 and 2) but the crystal structure changes to cubic AuBe 5 -type in PrCu 3 Pd 2 , NdCu 5-x Pd x (x=1 and 2) and SmCu 4 Pd. Substitution with Pd leads to lattice expansion and modifies the magnetic behavior. While PrCu 5 is known to be a van-Vleck paramagnet with a singlet ground state, PrCu 4 Pd and PrCu 3 Pd 2 show ferromagnetic-like behavior at low temperatures. SmCu 4 Pd orders ferromagnetically near 28 K in contrast to the antiferromagnetic nature of the parent SmCu 5 . The divalent nature of the Eu ions in EuCu 5 is retained in the ternary alloys, but the Curie temperature is reduced from 57 to 24.5 and 14.5 K in EuCu 4 Pd and EuCu 3 Pd 2 , respectively, inferred from the location of peak in the heat capacity of these two compounds. The magnetic hyperfine field at the Eu nucleus measured with 151 Eu Moessbauer spectroscopy in the ternary Eu-alloys is comparable to that in EuCu 5 . The magnetic behavior of NdCu 4 Pd is similar to that reported in NdCu 5 . The zero-field-cooled, low-field magnetization of NdCu 3 Pd 2 shows a region of diamagnetic behavior roughly between 21 and 4 K, but the field-cooled response is positive.

    Full Text Available CONTEXT: Ulcerative colitis is an inflammatory bowel disease involving superficial inflammation of the mucosa of the colon, rectum, and anus, sometimes including the terminal ileum. When in clinical activity, the disease is characterized by various daily evacuations containing blood, mucus and/or pus alternating periods of remission. OBJECTIVE: To compare nutritional parameters (dietary, biochemical and anthropometric among patients with ulcerative colitis followed up on an outpatient basis over a period of 1 year and during periods of intestinal inflammatory activity and remission. METHODS: Sixty-five patients were studied over a period of 1 year and divided into two groups: group 1 with inflammatory disease activity (n = 24, and group 2 without disease activity (n = 41. Anthropometric measures, biochemical parameters, quantitative food intake, and qualitative food frequency were analyzed. RESULTS: A significant reduction in body mass index and weight and in the intake of energy, proteins, lipids, calcium, iron and phosphorus was observed in the group with inflammatory activity (group 1 when compared to the period of clinical remission. The most affected food groups were cereals, legumes, oils, and fats. In contrast, in group 2 significant differences in triceps and sub scapular skin fold thickness, total protein, hemoglobin and hematocrit were observed between the first and final visit. Calcium and vitamin B6 intake, as well as the consumption of legumes, meat and eggs, and sugar and sweets, was significantly higher than on the first visit. CONCLUSION: Patients with ulcerative colitis followed up on an outpatient basis tend to be well nourished.     Full Text Available RESUMO: Descrevem-se, na pele de equídeos, lesões de natureza mecânico-traumática causadas por espinhos de Mimosa spp. Dentre as três espécies da planta identificadas como responsáveis pelas lesões, M. setosa estava presente em maior quantidade (80% e M. debilis e M. pudica encontravam-se em menor proporção na pastagem. Ocorreram três surtos de dermatite ulcerativa em períodos chuvosos de abril a maio de 2013, dezembro de 2013 a fevereiro de 2014 e abril a maio deste mesmo ano. Vinte e cinco equinos do Setor de Reprodução Animal da Universidade Federal Rural do Rio de Janeiro apresentaram, na pele, lesões ulcerativas com contornos irregulares, exsudato hemorrágico, recobertas por crostas. As lesões localizavam-se sobretudo nas regiões de quartela, boleto, articulação escapulo-umeral, lábios superior e inferior, focinho, narinas, bochechas e chanfro. Sete animais foram biopsiados e o exame histopatológico revelou ulceração da epiderme e infiltrado inflamatório constituído por macrófagos e neutrófilos, delimitado por tecido de granulação subjacente. Em alguns casos, foram observados microespículos das referidas plantas (tricomas hirsutos em meio à reação inflamatória. O diagnóstico de dermatite cutânea causada pela ação traumática da planta baseou-se na presença de Mimosa spp. na pastagem, nas características e localização das lesões na pele dos equinos, nos achados histopatológicos e na recuperação após a retirada dos animais do pasto.

    The study on the electric properties and magnetoresistance effect in the RCu 3 Mn 4 O 12 (where R is the rare-earth ion, Th) is carried out. It is established that all the compositions of the given series demonstrate the magnetoresistive effect, the value whereof at the liquid nitrogen temperature reaches 20% in the field 0.9 T. The increase in the magnetoresistance with the temperature decrease and high sensitivity to the weak magnetic fields at low temperatures indicate that this effect is intergranular. The peak of the magnetoresistance is identified near the Curie temperature (T C ). It is supposed that the degree of the magnetoresistance near the temperature of the magnetic ordering depends on the conditions of the samples synthesis and the effect of the intergranular interlayer on the transport properties of these compositions [ru

    Full Text Available The objective of this study was to evaluate the consensus of expert societies and published guidelines on the management of ulcerative colitis, and to compare with the experience of the authors, in order to standardize procedures that would help the reasoning and decision-making process of the physician. A search was performed in scientific literature, specifically in electronic databases: Medline/Pubmed, SciELO, EMBASE and Cochrane, and the following descriptors were used: ulcerative colitis, acute colitis, clinical treatment, surgery and randomized trial. It can be concluded that the goals of therapy in ulcerative colitis are clinical and endoscopic remission, deep, sustained remission without corticosteroids, prevention of hospitalizations and surgeries, and improved quality of life. The surgical indications are reserved for selected cases, ranging from medical intractability, complications (severe refractory acute colitis, toxic megacolon, perforation and hemorrhage and malignancy.     Full Text Available OBJETIVOS: descrever uma série de casos de colite ulcerativa inespecífica, com ênfase nas manifestações iniciais, propedêutica e intervalo entre os primeiros sintomas e o diagnóstico. MÉTODOS: análise retrospectiva de casos de colite ulcerativa inespecífica, cujos sintomas iniciaram antes de 20 anos de idade. Os pacientes foram selecionados nos Serviços de Gastroenterologia Pediátrica do Hospital das Clínicas da Universidade Federal de Minas Gerais e do Instituto Materno Infantil de Pernambuco. RESULTADOS: foram encontrados 29 pacientes com diagnóstico de colite ulcerativa inespecífica, sendo 15 (51,7% pacientes do sexo masculino. A idade ao início dos sintomas variou entre 0,3 a 15,8 anos. Os sintomas mais freqüentes ao diagnóstico foram diarréia (89,7% e dor abdominal (82,8%. A manifestação extra-intestinal mais comum foi artralgia/artrite (41,4%. A relação peso para a idade estava abaixo do percentil 3 em 13,8% dos pacientes. O intervalo de tempo entre o início dos sintomas e o diagnóstico variou entre 0,1 e 10,9 anos.     Full Text Available Introduction: pyoderma gangrenosum (PG is a rare and severe neutrophilic dermatosis asso- ciated with inflammatory bowel disease (IBD and other systemic diseases such as rheu- matoid arthritis and hematological malignancies. Diagnosis is based on clinical criteria and exclusion of other skin disorders. There is no gold standard for the treatment of PG; traditionally intravenous corticosteroids are used, but recently the use of drugs that in- hibit tumor necrosis factor alpha (TNF-alpha has changed the management of PG, showing great effectiveness.Case report: female patient, 23 years old, diagnosed with severe nonspecific ulcerative colitis (UC three years ago, undergoing treatment with oral mesalamine and azathioprine. She developed PG fourteen days after hospital discharge; hospitalization was due to worsening of intestinal disease symptoms. She was successfully treated using biological therapy after unfavorable evolution with corticosteroid therapy.Conclusion: PG, a rare extraintestinal manifestation of IBD of difficult resolution that has significant impact on patient quality of life. The use of biological therapy for PG has higher efficacy in the treatment of patients decreasing wound healing time and return to daily activities.     ALICE is a general-purpose detector that is designed to study the physics of quark-gluon plasma. The Time Projection Chamber (TPC) is one of the major detectors of ALICE. The TPC electronics consists of 4356 Front-end cards (FECs), which are controlled by 216 Readout Control Units (RCU). Each RCU connects to between 18 and 25 FECs using a multi-drop bus. In LHC Run1, the Readout Control Unit 1 (RCU1) performed even better than specification. However, in Run2 the energy of colliding beams is increased from 8 TeV to 14 TeV (maximum value) and higher luminosity, which leads to larger event size and higher radiation load on the electronics. As a solution, the Readout Control Unit 2 (RCU2) is designed to provide faster readout speed and improved radiation tolerance with respect to the RCU1. The RCU2 is conceptually similar to the RCU1 and it reuses the existing infrastructure and readout architecture of the TPC electronics.     This paper summarizes the overall program for inspection and maintenance of reactor structure and Reactivity Control Units (RCU) of HANARO during lifetime. The long-term plan for in-service inspection is introduced in the viewpoint of the structural integrity of reactor and RCU, and the operability of RCU mechanism. This program includes the list of components to be inspected, the schedule of inspection and maintenance, and the development of special tools and test rig that are required for the remote inspection and maintenance of reactor and RCU components. Preliminary results of the evaluation on the lifetime of RCU components are summarized based on the operation history since the installation of reactor. A test rig will be designed and constructed for the purposes of verifying the prolonged lifetime of RCU components being used, the performance of special tools, and the rehearsal of maintenance work as well. This paper proposes evaluation model for E-Learning platform in Riyadh City universities (RCU with Applied Geographic Information System (GIS. The E-Learning platform solution selection is a multiple criteria decision-making problem that needs to be addressed objectively taking into consideration the relative weights of the criteria for any organization. We formulate the quoted multi criteria problem as a decision hierarchy to be solved using GIS. AGIS-based evaluation index system and web-based evaluating platform were established. In this paper we will show the general evaluation strategy and some obtained results using our model to evaluate some existing commercial platforms.The results of evaluation model are outlined as follows: Total weights of the proposed framework in management feature is 20.25/25, in collaborative feature is 9.2/10, in adaption learning path is 6.8/10 and in interactive learning object is 5/5. The total weights of all features are 41.25/50. In this study an evaluation model was applied on Riyadh City universities like KSU, IMAMU, NAUSS, YU and FU. Then, the results were compared with each other. The total weighs of KSU was 41. While the total weights of FU, IMAMU, YU and NAUSS was 40, 37, 36 and 32, respectively. Evaluation process shows that the proposed framework satisfied the objectives with applied GIS.

    The readout electronics in PHOS and TPC - two of the major detectors of the ALICE experiment at the LHC - consist of a set of Front End Cards (FECs) that digitize, process and buffer the data from the detector sensors. The FECs are connected to a Readout Control Unit (RCU) via two sets of custom made PCB backplanes. For PHOS, 28 FECs are connected to one RCU, while for TPC the number is varying from 18 to 25 FECs depending on location. The RCU is in charge of the data readout, including reception and distribution of triggers and in moving the data from the FECs to the Data Acquisition System. In addition it does low level control tasks. The RCU consists of an RCU Motherboard that hosts a Detector Control System (DCS) board and a Source Interface Unit. The DCS board is an embedded computer running Linux that controls the readout electronics. Investigar los efectos del D-002, mezcla de seis alcoholes alifáticos primarios de alto peso molecular, obtenida de la cera de abejas (Apis mellifera, sobre la colitis ulcerativa (CU inflamatoria severa inducida por sulfato de dextrano (DSS y etanol en ratas (Ratus ratus. Materiales y métodos. Las ratas se distribuyeron aleatoriamente en seis grupos: un control cero al que no se provocó daño, y cinco a los que se les indujo la CU: un control negativo (vehículo, tres tratados con D-002 (25, 100 y 400 mg/kg y un control positivo con sulfazalacina (200 mg/kg (sustancia de referencia. Se cuantificaron las manifestaciones clínicas (variación del peso corporal, presencia de diarrea y de sangrado rectal, el puntaje de daño macroscópico e histológico, y la actividad de mieoloperoxidasa (MPO. Resultados. El tratamiento oral con D-002 (25, 100 y 400 mg/kg previno significativamente la disminución del peso corporal. La dosis de 400 mg/kg redujo la presencia de diarreas y sangrado rectal, aunque su comparación con el control negativo solo alcanzó significación estadística sobre las diarreas. El D-002 (25, 100 y 400 mg/kg redujo significativamente el puntaje de las lesiones macroscópicas (40,0; 43,3 y 47,2% de inhibición, respectivamente, el puntaje de daño histológico (31,5; 53,7 y 67,1% de inhibición, respectivamente y la actividad de MPO (73,2; 83,6 y 85,0% de inhibición, respectivamente, comparado con el grupo control negativo. La sulfazalacina redujo significativamente todas las variables estudiadas. Conclusiones. El D-002 (25, 100 y 400 mg/kg protegió significativamente la mucosa colónica en ratas con CU inflamatoria severa inducida por DSS y etanol.

    Cocaine dependence has been associated with blunted dopamine and norepinephrine signaling, but it is unknown if recreational cocaine use is also associated with alterations of catecholamine systems. Prepulse inhibition (PPI) of the acoustic startle response-a measure of sensorimotor gating-is highly sensitive for manipulations of the catecholamine system. Therefore, we investigated whether relatively pure recreational users (RCU) and dependent cocaine users (DCU) display alterations of PPI, startle reactivity, and habituation. Moreover, the influences of methylenedioxymethamphetamine and cannabis co-use, craving, and attention-deficit/hyperactivity disorder (ADHD) symptoms on startle measures were examined. In 64 RCU, 29 DCU, and 66 stimulant-naïve control subjects, PPI of acoustic startle response, startle reactivity, habituation, ADHD symptoms, and cocaine craving were assessed. Drug use of all participants was controlled by hair and urine toxicologies. Both RCU and DCU showed increased PPI in comparison with control participants (Cohen's d=.38 and d=.67, respectively), while RCU and DCU did not differ in PPI measures (d=.12). No significant group differences were found in startle reactivity or habituation measures. In cocaine users, PPI was positively correlated with cumulative cocaine dose used, craving for cocaine, and ADHD symptoms. Users with a diagnosis of ADHD and strong craving symptoms displayed the highest PPI levels compared with control subjects (d=.78). The augmented PPI in RCU and DCU suggests that recreational use of cocaine is associated with altered catecholamine signaling, in particular if ADHD or craving symptoms are present. Finally, ADHD might be a critical risk factor for cocaine-induced changes of the catecholamine system. Copyright © 2013 Society of Biological Psychiatry. Published by Elsevier Inc.   13. Radiation tolerance studies using fault injection on the Readout Control FPGA design of the ALICE TPC detector

    Single Event Upsets (SEUs) are a major concern for the TPC Readout Control Unit (RCU) of the ALICE experiment. A SEU is defined as a radiation related bit-flip in a memory cell, and a SEU in the onboard SRAM based FPGA of the RCU may lead to corrupted data or, even worse, a system malfunction. The latter situation will affect the operation of the ALICE detector since it causes a premature end of data taking. Active partial reconfiguration is utilized in a dedicated reconfiguration solution on the RCU, and this makes it possible to implement fault injection. Fault injection means inserting bit flips in the configuration memory of the FPGA in a controlled laboratory environment. This paper presents the results of the fault injection study and shows how this result can be combined with SEU measurements to estimate the functional failure rate as a function of luminosity.

  15. Epidemiologia e distribuição de lesões extrarrenais de uremia em 161 cães

    Full Text Available Resumo: Com o objetivo de determinar a epidemiologia e as características morfológicas, incluindo a localização anatômica, das lesões extrarrenais de uremia, bem como determinar as principais lesões do sistema urinário associadas à ocorrência de uremia, foram revisados os protocolos de necropsias de cães realizadas no Laboratório de Patologia Veterinária da Universidade Federal de Santa Maria de janeiro de 1996 a dezembro de 2012 (17 anos. Nesse período foram necropsiados 4.201 cães, sendo que 161 (3,8% apresentaram lesões extrarrenais de uremia. Em 134 cães (83,2% foram descritos sinais clínicos associados à uremia. As lesões extrarrenais mais frequentes, em ordem decrescente, foram: gastrite ulcerativa e hemorrágica (56,5%, mineralização de tecidos moles (55,9%, edema pulmonar (47,2%, estomatite e/ou glossite ulcerativa (30,4%, endocardite/trombose atrial e aórtica (28,6%, hiperplasia das paratireoides (9,3%, osteodistrofia fibrosa (8,1%, anemia (6,2%, laringite ulcerativa (5%, enterite ulcerativa/hemorrágica (3,7%, esofagite fibrinonecrótica (1,9% e pericardite fibrinosa (1.9%. Na maioria dos casos as lesões extrarrenais de uremia foram decorrentes de azotemia prolongada por lesões renais graves, sendo as mais prevalentes a nefrite intersticial e a glomerulonefrite.

    Full Text Available Foram avaliadas as frações da proteína e dos carboidratos, a degradabilidade ruminal efetiva (DE da matéria seca (MS e proteína bruta (PB, a digestibilidade ruminal in vitro (DRIV da MS e PB, a digestibilidade intestinal in vitro (DIIV da proteína nãodegradada no rúmen (PNDR e os perfis de aminoácidos (AA e de ácidos graxos (AG do resíduo de cervejaria úmido (RCU e fermentado (RCF. O RCF foi obtido pelo processo de fermentação microbiana do RCU. Para determinar a DE da MS e PB do RCU e RCF, foram utilizados três novilhos da raça Holandesa, portadores de cânula ruminal. A DIIV da PNDR foi obtida pelo método de três estágios. Os dados obtidos para DE da MS e PB foram submetidos à análise de variância, em delineamento inteiramente casualizado. A fração A da PB do RCU foi de 7,9% e do RCF de 13,1% da PB. A DE da PB a 5% h-1 não diferiu (p IV da PB do RCF foi de 8,7% e as DIIV da PNDR do RCU e do RCF foram de 70,5 e 72,5%, respectivamente. Os perfis de AA e AG do RCU e RCF foram similares.     Full Text Available Esta investigación tuvo como objetivo verificar la asociación existente entre las variables sociodemográficas y los procesos de enfrentamiento al estrés de los individuos portadores de Rectocolitis Ulcerativa Idiopática atendidos en el Ambulatorio de Enfermedades Inflamatorias de Colon del Hospital de las Clínicas de la Facultad de Medicina de la Universidad de San Pablo. Del total de 100 individuos participantes del estudio se verificó que la variable practicante de creencia religiosa presentó resultado estadísticamente significativo para los factores fuga/ esquiva, aceptación de responsabilidad y alejamiento. Con este estudio se pudo concluir que la creencia religiosa favorece la utilización de diversos factores de enfrentamiento para el manejo de situaciones adversas como la enfermedad y de este modo contribuir para mejorar la calidad de vida de los portadores de la enfermedad aquí estudiada.The objective of this research was verify any association between socio-demographic characteristics and and stress coping strategies among ulcerative colitis patients. This study took place in the Inflammatory Diseases Outpatient Clinic of Hospital das Clinicas of the University of São Paulo Medical School. The 100 individual participants of this study verified that the variable of those who practiced their religious faith presented statistically significant results in the factors disdainful escapement, acceptance of responsibility and separation. Conclusion: individuals of this study who practiced their religious faith predominantly used disdainful escapement, acceptance of responsibility, self-control as a way of coping with adverse situations such as this disease, contributed to an improvement in their quality of life.

    Full Text Available Rationale: Crohn's disease and non-specific ulcerative colitis are part of inflammatory bowel diseases. They have a chronic evolution, leading to important repercussions on patients’ quality of life. Measuring this subjective parameter requires an evaluation tool in clinical trials and health programs. The “Inflammatory Bowel Disease Questionnaire” is an American instrument of McMaster University, which had its reproducibility and validity determined in studies in other countries as a measure of the quality of life in IBD. Objective: To evaluate the quality of life of patients with inflammatory bowel disease through the Inflammatory Bowel Disease Questionnaire, and to correlate the results with sociodemographic data of the patients. Methods: This is a prospective cross-sectional study carried out with 58 patients; the patients’ follow-up was conducted at the outpatient clinic of Coloproctology. Results: Among the 58 patients evaluated, 70.1% had DC, 62.1% were women, the mean age was 46.08 years, 96.6% were non-smokers, and 24.1% were submitted to surgery for the underlying disease. 43% were in a combination therapy scheme, 44% in monotherapy, and 12% were not using medication. Significant change in quality of life was observed in patients taking prednisone. Conclusion: The patients with better quality of life are those who were taking prednisone. There was no other correlation with significance in the patients’ quality of life.     Full Text Available Diversos estudos têm demonstrado que alguns indivíduos, ante a impossibilidade de representar mentalmente seus conflitos, expressam-nos com o corpo, por meio de sintomas psicossomáticos. Também tem sido investigado como situações de estresse, ansiedade e depressão podem influir no curso e na evolução de determinadas doenças, incluindo as inflamatórias do intestino. Esta pesquisa reuniu trinta pacientes portadores de retocolite ulcerativa e doença de Crohn, que foram submetidos a três testes psicológicos: Inventário Beck de Ansiedade, Inventário Beck de Depressão e Inventário de Sintomas de Estresse para Adultos de Lipp. Posteriormente, nove deles foram selecionados para realização de um trabalho psicoterapêutico breve em grupo. Os testes revelaram que a condição emocional, embora não possa ser considerada diretamente causadora, pode afetar as defesas do sujeito, agravando a patologia. O trabalho psicoterapêutico revelou situações emocionais até então não percebidas, mas presentes nas histórias de vida e manifestadas sintomaticamente por meio de somatizações.Muchos estudios han demostrado que algunos individuos, frente a la imposibilidad de representar mentalmente sus conflictos, los expresan con el cuerpo, a través de síntomas psicosomáticos.     Highlights: • Antiferromagnetic material RCu{sub x}Ge{sub 2} of high purity was prepared. • Large MCE as −10.2 J/kg K and −10.5 J/kg K for RCu{sub x}Ge{sub 2} (Ho, Er) was obtained for field change of 0–50 kOe. • The RCu{sub x}Ge{sub 2} compounds with variable x had different transition temperature which made them suitable for ‘table-like’ magnetocaloric refrigerant. - Abstract: Magnetic properties and magnetocaloric effect (MCE) of HoCu{sub 0.33}Ge{sub 2} and ErCu{sub 0.25}Ge{sub 2} compounds have been investigated. The compounds were determined to be antiferromagnetic with the Néel temperatures T{sub N} = 9 K and 3.9 K, respectively. The critical transition magnetic fields for the metamagnetic transition from antiferromagnetic to ferromagnetic state below T{sub N} were determined to be 10 kOe for HoCu{sub 0.33}Ge{sub 2} at 5 K and 6 kOe for ErCu{sub 0.25}Ge{sub 2} at 2 K. Large MCE with the maximal values of magnetic entropy changes (ΔS{sub M}) as −10.2 J/kg K at 10.5 K were found in HoCu{sub 0.33}Ge{sub 2} for field changes of 0–70 kOe and −10.5 J/kg K at 5.5 K in ErCu{sub 0.25}Ge{sub 2} for field changes of 0–50 kOe, respectively. The large ΔS{sub M} around T{sub N} as well as no hysteresis loss made RCu{sub x}Ge{sub 2} competitive candidates as low temperature magnetic refrigerant.

    Superconductivity can be induced at high temperatures in Pb 2 Sr 2 RCu 3 O 8 (R - rare earth) by partially doping Ca 2+ for R 3+ . In order to understand the interplay between magnetism and superconductivity, the magnetic properties of the parent compounds, Pb 2 Sr 2 RCu 3 O 8 , have been studied. The work presented here includes magnetic susceptibility and specific heat measurements on R=Dy and extends the previous studies on R=Ce, Pr, Tb, Ho and Er. Specific heat experiments suggest that the Dy ions order antiferromagnetically with an ordering temperature of 1.3K. The magnetic susceptibility data are in good agreement with the susceptibility calculated using crystal field parameters that are extrapolated from previous modeling of the R=Er and Ho analogs of this series

    ABSTRACT

    Uniaxial compressive strength (UCS deals with materials' to ability to withstand axially-directed pushing forces and especially considered to be rock materials' most important mechanical properties. However, the UCS test is an expensive, very time-consuming test to perform in the laboratory and requires high-quality core samples having regular geometry. Empirical equations were thus proposed for predicting UCS as a function of rocks' index properties. Analytical hierarchy process and multiple regression analysis based methodology were used (as opposed to traditional linear regression methods on data-sets obtained from carbonate rocks in NE Turkey. Limestone samples ranging from Devonian to late Cretaceous ages were chosen; travertine-onyx samples were selected from morphological environments considering their surface environmental conditions Test results from experiments carried out on about 250 carbonate rock samples were used in deriving the model. While the hierarchy model focused on determining the most important index properties affecting on UCS, regression analysis established meaningful relationships between UCS and index properties; 0. 85 and 0. 83 positive coefficient correlations between the variables were determined by regression analysis. The methodology provided an appropriate alternative to quantitative estimation of UCS and avoided the need for tedious and time consuming laboratory testing

  4. Impaired emotional empathy and related social network deficits in cocaine users.

    Chronic cocaine users consistently display neurochemical and functional alterations in brain areas involved in social cognition (e.g. medial and orbitofrontal cortex). Although social functioning plays a crucial role in the development and treatment of drug dependence, studies investigating social cognition in cocaine users are lacking. Therefore, we investigated mental perspective taking ('theory of mind') and emotional and cognitive empathy in recreational (RCU) and dependent (DCU) cocaine users. Furthermore, we related these measures to real-life indicators of social functioning. One-hundred cocaine users (69 RCU, 31 DCU) and 68 stimulant-naïve healthy controls were tested with the Multifaceted Empathy Test (MET), Movie for the Assessment of Social Cognition (MASC) and Reading the Mind in the Eyes Test (RMET). The Social Network Questionnaire was conducted to assess social network size. Furthermore, participants provided information on committed criminal offenses. RCU and DCU showed less emotional empathy compared to controls (MET), whereas cognitive empathy was not impaired (MET, RMET). Additionally, DCU made more errors in mental perspective taking (MASC). Notably, cocaine users committed more criminal offenses and displayed a smaller social network and higher cocaine use was correlated with less social contacts. Diminished mental perspective taking was tentatively correlated with more intense cocaine use as well. Finally, younger age of onset of cocaine use was associated with more pronounced empathy impairment. In conclusion, social cognition impairments in cocaine users were related to real-life social functioning and should therefore be considered in therapy and prevention strategies. © 2013 Society for the Study of Addiction.

  7. Home tracheotomy mechanical ventilation in patients with amyotrophic lateral sclerosis: causes, complications and 1-year survival.

    Home tracheotomy mechanical ventilation (HTMV) can prolong survival in patients with amyotrophic lateral sclerosis (ALS) when non-invasive ventilation (NIV) fails, but knowledge about HTMV is scarce. The aim of this study was to determine the causes of tracheotomy and the main issues of 1-year HTMV in a cohort of patients with ALS. A prospective study of all patients needing HTMV was performed in a referral respiratory care unit (RCU) from April 2001 to January 2010. Patients' informed decisions about HTMV were fully respected. Caregivers were trained and could telephone the RCU. Hospital staff made home visits. All patients (n=116) agreed to participate and a tracheotomy was needed for 76, mainly due to bulbar dysfunction. Of the 38 who had a tracheotomy, in 21 it was performed in an acute setting and in 17 as a non-emergency procedure. In 19 patients the tracheotomy was related to the inadequacy of mechanically assisted coughing (MAC) to maintain normal oxygen saturation. During HTMV, 19 patients required hospitalisation, 12 with respiratory problems. The 1-year survival rate was 78.9%, with a mean survival of 10.39 months (95% CI 9.36 to 11.43). Sudden death was the main cause of death (n=9) and only one patient died from respiratory causes. No predictive factors for survival were found. Besides NIV inadequacy, the ineffectiveness of mechanically assisted coughing appears to be a relevant cause of tracheotomy for patients with ALS with severe bulbar dysfunction. Patients choosing HTMV provided by a referral RCU could have a good 1-year survival rate, respiratory problems being the main cause of hospitalisation but not of death.

    Full Text Available Em 1981 Glotzer et al. descreveram um tipo de proctocolite, semelhante à retocolite ulcerativa, limitado ao segmento excluído do trânsito fecal, na ausência de doença intestinal inflamatória. O presente estudo tem por finalidade avaliar as alterações ocorridas no cólon após sua exclusão do trânsito, com o fim de se estabelecer um modelo em ratos que permita estudar a doença. Utilizaram-se 35 ratos Wistar-PUCPR, machos com 120 dias de idade, divididos em 4 grupos. Sob anestesia inalatória procedeu-se à laparotomia mediana, secção transversa do cólon esquerdo e colectomia de 0,5 cm par se ter o padrão inicial (Mo. Seguiu-se o fechamento do coto distal, colostomia terminal com maturação precoce do coto proximal e laparorrafia. Realizaram-se as verificações nos seguintes tempos: grupo A após uma semana, grupo D após 2 semanas, grupo B após 4 semanas e grupo C após 8 semanas. Avaliou-se o cólon excluído macro e microscópicamente (Mf analisando-se: a reação inflamatória, a concentração de colágeno e o comportamento das células caliciformes. Comparadas os momentos Mo e Mf registrou-se a presença de úlceras em 11 cólons (p=0,0010 não relacionadas com o tempo de exclusão. Reação inflamatória agudo-crônica ou crônica discreta esteve presente em todos os tempos. Em todos os cólons observou-se diminuição significante da espessura da parede. No Mo predominou o colágeno tipo I (p=0,008 enquanto que no Mf o predomínio foi do colágeno III (p=0,008. Foi constante a diminuição do colágeno total, a perda de colágeno I e o aumento de colágeno III. Não houve mudança significante do percentual de área ocupada por células caliciformes. Conclui-se que a exclusão do cólon distal, de até 8 semanas, no rato por colostomia determina: atrofia do cólon e o aparecimento de lesões ulceradas superficiais com reação inflamatória discreta.

    The ALICE Time Projection Chamber (TPC) is the main tracking detector of the central barrel of the ALICE (A Large Ion Collider) Experiment at the Large Hadron Collider (LHC), being constructed at CERN, Geneva. It is a 88 m$^{3}$ cylinder filled with gas and divided into two drift regions by the central electrode located at its axial center. The readout chambers of the TPC are multi-wire proportional chambers with cathode pad readout. About 570 000 pads are read-out by an electronics chain of amplification, digitalization and pre-processing. One of the challenges in designing the TPC for ALICE is the design of Front End Electronics (FEE) to cope with the data rates and the channel occupancy.     Full Text Available Necrotizing ulcerative gingivitis is a relatively uncommon periodontal disease, characterized by ulceration, necrosis, pain and gingival bleeding. Factors often related to its occurrence include stress and systemic viral infections, such as those caused by cytomegalovirus and Epstein-Barr virus type 1, the latter being also considered the causative agent of infectious mononucleosis. This article aims to describe a clinical case of a female patient who presented with necrotizing ulcerative gingivitis associated with a clinical picture of infectious mononucleosis, as well as to review the literature concerning a possible correlation between these pathologies. This patient presented to our health care facility with necrotizing ulcerative gingivitis accompanied by lymphadenopathy, fever and prostration, after laboratory tests, Epstein-Barr virus type 1 infection was confirmed, as well as the co-occurrence of pathologies: necrotizing ulcerative gingivitis and infectious mononucleosis. Symptom remission in both disorders also occurred concomitantly, after instruction in plaque control measures and palliative medication for control of systemic symptoms.     Full Text Available Introduction. Retrocaval ureter is a rare congenital anomaly. Open surgery was the classic treatment for this condition. Laparoscopy is currently an admitted procedure to treat many urological diseases. The objective of our study is to present our experience and discuss the safety and the feasibility of transperitoneal laparoscopic pyelopyelostomy for treatment of retrocaval ureter (RCU. Materials and Methods. Three symptomatic patients underwent laparoscopic repair for RCU in our department. The diagnosis was suspected on the computed tomography scan (CT and confirmed on ascending pyelography. After placement of a JJ stent, and, using the transperitoneal approach, the retro peritoneum was exposed; the ureter was identified in both sides of the vena cava. The retrocaval segment was entirely mobilized and pulled from behind of the vena cava after section of renal pelvis. A pyelopyelostomy was done in a normal anatomic position. Results. All operations were achieved laparoscopically without conversion to open surgery. The mean operative time was 140 minutes (110–190. No intraoperative complication occurred. Blood loss was less than 50 mL in all patients. The mean hospital stay was 5 days (4–6 days. All patients were symptom-free after surgery and had reduction of hydronephrosis in control imagery. Conclusion. Laparoscopy seems safe, feasible, and reproducible in managing retrocaval ureter.

To study episodic memory in OCD patients, we examined the directed forgetting effect. Patients with OCD and healthy control participants were given a list of 24 emotionally neutral everyday words (12 remember [R]-cued words and 12 forget [F]-cued words under two conditions: List and Item. The results of our study showed that OCD patients recalled a number of F-cued words similar to that for controls and relatively fewer R-cued words than controls under both List and Item conditions. Consequently, the directed forgetting effect was smaller in OCD patients than controls. Our results demonstrated that both selective encoding and retrieval inhibition processes are impaired in OCD, and we suggest that recall of unfavorable items to be forgotten intruded into necessary items to be remembered. This impairment in episodic memory may partially account for some of the unique clinical symptoms of OCD.Keywords: episodic memory, retrieval inhibition, selective encoding

    Capturing the full range of climatic diversity in a reserve network is expected to improve the resilience of biodiversity to climate change. Therefore, a study on systematic conservation planning for climatic diversity that explicitly or implicitly hypothesizes that regions with higher climatic diversity will support greater biodiversity is needed. However, little is known about the extent and generality of this hypothesis. This study utilized the case of Yunnan, southwest China, to quantitatively classify climatic units and modeled 4 climatic diversity indicators, including the variety of climatic units (VCU), rarity of climatic units (RCU), endemism of climatic units (ECU) and a composite index of climatic units (CICD). We used 5 reliable priority conservation area (PCA) schemes to represent the areas with high biodiversity conservation value. We then investigated the spatial relationships between the 4 climatic diversity indicators and the 5 PCA schemes and assessed the representation of climatic diversity within the existing nature reserves. The CICD exhibited the best performance for indicating high conservation value areas, followed by the ECU and RCU. However, contrary to conventional knowledge, VCU did not show a positive association with biodiversity conservation value. The rarer or more endemic climatic units tended to have higher reserve coverage than the more common units. However, only 28 units covering 10.5% of the land in Yunnan had more than 17% of their areas protected. In addition to climatic factors, topography and human disturbances also significantly affected the relationship between climatic diversity and biodiversity conservation value. This analysis suggests that climatic diversity can be an effective surrogate for establishing a more robust reserve network under climate change in Yunnan.     Full Text Available This report offers a macroscopic and histological description of ulcerative gastric lesions in Humboldt penguins associated with the presence of Contracaecum pelagicum. Parasites of this genus have been isolated from different species of marine mammals and birds, including penguins, demonstrating its capacity to generate ulcerative lesions in the gastrointestinal tracts of many hosts. A bibliographic review revealed no publications on gastric ulceration by nematodes in Humboldt penguins. Studies carried out in Chile concerning gastrointestinal parasitism by nematodes of the Contracaecum genus in S. humboldti only established the parasite's taxonomy. This is the first pathological description of a parasitic ulcerative manifestation in the stomach of S. humboldti in the world. The Humboldt penguin is a vulnerable species.     Full Text Available O cavalo, dado o seu meio ambiente, está sujeito a afecções frequentes da córnea e da conjuntiva, tecidos oculares bastante expostos a bactérias e fungos, principalmente Aspergillus spp. e Fusarium spp. As ceratites ulcerativas bacterianas e fúngicas, bem como as ceratites fúngicas não ulcerativas, caracterizadas principalmente pelo abscesso estromal, são frequentes nessa espécie. Ocorrida a lesão inicial, perpetua-se um ciclo vicioso, com liberação de citocinas inflamatórias, que desencadeiam uma rápida e severa infiltração corneal por células polimorfonucleares. A córnea torna-se sujeita à destruição por enzimas proteolíticas liberadas pelos micro-organismos e por células inflamatórias, capazes de desencadear a dissolução estromal e a perfuração do bulbo ocular. O tratamento clínico para a resolução da doença corneal e o controle da uveíte reflexa deve ser agressivo e associado, muitas das vezes, à terapia cirúrgica. Este artigo discorre sobre a fisiopatologia e o tratamento da ceratomicose em equinos.Environmental and behavioral factors make horses susceptible to corneal and conjunctival lesions, since these structures are constantly exposed to bacteria and fungi specially Aspergillus spp. and Fusarium spp. Bacterial and fungal ulcerative keratitis, as well as non-ulcerative fungal keratitis such as stromal abscess, are frequent in horses. A "cascade" effect follows the initial lesion which triggers the release of inflammatory cytokines followed by an acute and severe infiltrate of polymorphonuclear cells in the cornea. The cornea becomes susceptible to the activity of proteolytic enzymes released by microorganisms and polymorphonuclear cells, resulting in stromal degradation and ocular perforation. The medical treatment targeting the corneal disease and the controlling of reflexive uveitis should be aggressive and surgical therapy should be associated in most of the cases. Esta condição inflamatória, ao mimetizar a doença de Crohn e a colite ulcerativa, pode resultar em diagnóstico e tratamento inapropriados. Este estudo prospectivo objetivou rever os efeitos da derivação fecal e caracterizar a evolução após a restauração do trânsito intestinal. Foram avaliados treze pacientes sem doença inflamatória intestinal e submetidos previamente à uma colostomia. Os achados da endoscopia com biópsia colo-retal, dos segmentos proximal e distal desfuncionalizados, evidenciaram uma inflamação inespecífica da mucosa e da submucosa, muito semelhante aquela da colite ulcerativa (p<0.01. Depois do fechamento da colostomia observou-se a regressão das alterações endoscópicas e histopatológicas.     Full Text Available Se presenta el caso de un paciente varón de 31 años, con diagnóstico de infección por el virus de inmunodeficiencia humana (VIH sin tratamiento antirretroviral, presentó lesiones ulcerativas en la región genital que luego progresaron por todo el cuerpo, se le diagnosticó sífilis secundaria. Fue tratado con penicilina benzatínica, con lo cual las lesiones desaparecieron. Se discute las pruebas diagnósticas realizadas y la terapéutica empleada. La coinfección por Treponema pallidum y por el VIH está presente en la población general y en especial en poblaciones de riesgo, como los hombres que tienen sexo con hombres y clientes de trabajadoras sexuales, esta situación puede afectar la presentación inicial así como el curso de la enfermedad.

    Full Text Available Relatou-se um caso clínico de dermatite multifatorial em cão, envolvendo Demodex canis, Sporothrix schenckii e bactérias dos gêneros Staphylococcus e Streptococcus. O animal, macho, da raça Pit Bull, com um ano e três meses de idade, apresentou lesões úmidas, hiperêmicas, descamativas e ulcerativas nas regiões do pescoço e do tórax. Exames laboratoriais confirmaram o diagnóstico de infecção concomitante por fungo, bactéria e parasito. A terapia antimicrobiana com moxidectina, itraconazol e amoxicilina foi efetiva, com remissão dos sinais clínicos após 30 dias de tratamento. Este relato salienta a importância do diagnóstico de certeza, que foi determinante para estabelecimento da terapêutica adequada e recuperação do animal.

    The fraction of J /ψ mesons which come from B -meson decay, FB →J /ψ, is measured for J /ψ rapidity 1.2 0 in p +p and Cu+Au collisions at √{sNN} = 200 GeV with the PHENIX detector. The extracted fraction is FB →J /ψ=0.025 ±0.006 (stat) ± 0.010(syst) for p +p collisions. For Cu+Au collisions, FB →J /ψ is 0.094 ± 0.028(stat) ± 0.037(syst) in the Au-going direction (-2.2 RCuAu, of B mesons in Cu+Au collisions is consistent with binary scaling of measured yields in p +p at both forward and backward rapidity.

    Full Text Available Neste trabalho, é descrito um caso de estenose hipertrófica de piloro diagnosticado por gastroscopia em um eqüino Quarto de Milha, de quatro anos de idade, que apresentava inapetência, emagrecimento progressivo, cólica, bruxismo e sialorréia. A gastroduodenite ulcerativa crônica e os espasmos prolongados foram as supostas causas da hipertrofia da musculatura circular do piloro, causando a estenose. O tratamento com substâncias antiulcerativas é paliativo e o acesso cirúrgico ao piloro no animal adulto é complexo, sendo o prognóstico duvidoso.A case of hypertrophic pyloric stenosis diagnosed by gastroscopy was reported in a 4-years-old Quarter Horse gelding, with signs of inappetence, chronic weight loss, colic, bruxism and sialorrhea. Gastroduodenal ulceration and spasms were supposed as causes of pyloric muscular hypertrophy and stenosis. Antiulcerative medications are merely palliative and the complex surgical approach to the site of stenosis makes the prognostic to be doubtful.

    Full Text Available La cistitis intersticial se define como un síndrome clínico caracterizado por un incremento de la frecuencia urinaria, urgencia miccional y/o dolor abdominal o perineal en ausencia de infección urinaria o enfermedad conocida del aparato urinario. Se divide de acuerdo a los hallazgos citoscópicos en ulcerativa o no. La base del diagnóstico es clínica apoyada en scores de probabilidad y pruebas invasivas. Con la comprensión de la fisiopatología se ha desarrollado un abanico de posibilidades terapéuticas. Comunicamos el caso de una paciente sintomática por cuatro años con diagnóstico de cistitis intersticial con úlcera de Hunner por cistoscopia y excelente respuesta a la amitriptilina. Es una enfermedad que produce considerable alteración de la calidad de vida, por lo que es importante que los médicos estén familiarizados con este cuadro.

    Full Text Available Foi estudada uma doença em bovinos e ovinos caracterizada por lesões ulcerativas e granulomatosas da pele dos membros. Os estudos epidemiológicos e patológicos permitiram concluir que essas lesões são causadas pelos espinhos de Mimosa pudica (Leg. Mimosoideae. A doença foi observada somente em pastagens acentuadamente infestadas e os animais se recuperaram rapidamente após retirados destes pastos. Nos ovinos as lesões atingiam partes mais altas dos membros do que nos bovinos, devido ao seu menor porte.A condition of cattle and sheep characterized by ulcerous and granulomatous skin lesions of the legs was studied. Epidemiological data and the nature of the lesions indicate that they are caused by the thorns of Mimosa pudica (Leg. Mimosoideae. The disease was observed only in pastures largely infested by the plant, and animals recover quickly when moved from them.

    Full Text Available Estimating maximum possible rainfall is of great value for flood prediction and protection, particularly for regions, such as Canada, where urban and fluvial floods from extreme rainfalls have been known to be a major concern. In this study, a methodology is proposed to forecast real-time rainfall (with one month lead time using different number of spatial inputs with different orders of lags. For this purpose, two types of models are used. The first one is a machine learning data driven-based model, which uses a set of hydrologic variables as inputs, and the second one is an empirical-statistical model that employs the multi-criteria decision analysis method for rainfall forecasting. The data driven model is built based on Artificial Neural Networks (ANNs, while the developed multi-criteria decision analysis model uses Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS approach. A comprehensive set of spatially varying climate variables, including geopotential height, sea surface temperature, sea level pressure, humidity, temperature and pressure with different orders of lags is collected to form input vectors for the forecast models. Then, a feature selection method is employed to identify the most appropriate predictors. Two sets of results from the developed models, i.e., maximum daily rainfall in each month (RMAX and cumulative value of rainfall for each month (RCU, are considered as the target variables for forecast purpose. The results from both modeling approaches are compared using a number of evaluation criteria such as Nash-Sutcliffe Efficiency (NSE. The proposed models are applied for rainfall forecasting for a coastal area in Western Canada: Vancouver, British Columbia. Results indicate although data driven models such as ANNs work well for the simulation purpose, developed TOPSIS model considerably outperforms ANNs for the rainfall forecasting.     Full Text Available Descreve-se aqui o encontro do helminto Stephanofilaria spp., em dermatite nodular ulcerativa em seres humanos, sendo as ulceras pesquisadas de dimensões variadas e com alguns anos de existência. Acredita-se que, a presença desse parasita, que afeta diversas espécies de animais domésticos e silvestres, possa agravar o quadro com as lesões produzidas nos vasos venosos e linfáticos, reduzindo a circulação, retardando a cicatrização. O ciclo deste parasita no ser humano, também é descrito.This work describes the occurrence of the helminth Stephanofilaria spp in ulcerative nodular dermatitis in humans. The ulcers observed presented a variety of sizes and some years of existence. It is believed that the presence of this parasite, which affects several domestic and wild animal species, may harm human health through lesions produced in venous and lymphatic vessels that reduce blood circulation and retard the healing process. The cycle of this parasite in humans is also described.

  12. An integrated autonomous rendezvous and docking system architecture using Centaur modern avionics

    The avionics system for the Centaur upper stage is in the process of being modernized with the current state-of-the-art in strapdown inertial guidance equipment. This equipment includes an integrated flight control processor with a ring laser gyro based inertial guidance system. This inertial navigation unit (INU) uses two MIL-STD-1750A processors and communicates over the MIL-STD-1553B data bus. Commands are translated into load activation through a Remote Control Unit (RCU) which incorporates the use of solid state relays. Also, a programmable data acquisition system replaces separate multiplexer and signal conditioning units. This modern avionics suite is currently being enhanced through independent research and development programs to provide autonomous rendezvous and docking capability using advanced cruise missile image processing technology and integrated GPS navigational aids. A system concept was developed to combine these technologies in order to achieve a fully autonomous rendezvous, docking, and autoland capability. The current system architecture and the evolution of this architecture using advanced modular avionics concepts being pursued for the National Launch System are discussed.

    In this paper we present the front end electronics for the time projection chamber (TPC) of the ALICE experiment. The system, which consists of about 570000 channels, is based on two basic units: (a) an analogue ASIC (PASA) that incorporates the shaping-amplifier circuits for 16 channels; (b) a mixed-signal ASIC (ALTRO) that integrates 16 channels, each consisting of a 10-bit 25-MSPS ADC, the baseline subtraction, tail cancellation filter, zero suppression and multi-event buffer. The complete readout chain is contained in front end cards (FEC), with 128 channels each, connected to the detector by means of capton cables. A number of FECs (up to 25) are controlled by a readout control unit (RCU), which interfaces the FECs to the data acquisition (DAQ), the trigger, and the detector control system (DCS) . A function of the final electronics (1024 channels) has been characterized in a test that incorporates a prototype of the ALICE TPC as well as many other components of the final set-up.     Full Text Available RESUMO A estenose esofágica benigna é uma afecção rara em pequenos animais, comumente secundária a esofagites ulcerativas. O refluxo gastroesofágico, frequente durante procedimentos anestésicos, é a principal causa de esofagite grave, com consequente formação de cicatriz esofágica. O presente trabalho tem por objetivo descrever dois casos de estenose esofágica ocorrentes após ovário-histerectomia, com destaque para os procedimentos diagnósticos realizados. Em ambos os casos, a combinação dos sinais clínicos sugestivos e os achados de esofagograma e esofagoscopia foram determinantes. No primeiro caso, devido ao tempo avançado de desenvolvimento dos sinais clínicos, o paciente veio a óbito antes mesmo que a intervenção direta da região de estenose fosse realizada.     Full Text Available Este estudo objetivou avaliar o efeito da fisioterapia nos parâmetros ventilatórios e na dinâmica tóraco-abdominal de crianças respiradoras bucais. Dez crianças respiradoras bucais foram submetidas a um programa de reeducação respiratória e correção postural durante 20 sessões de exercícios de alongamento e fortalecimento muscular sobre a bola suíça, estimulação diafragmática e alongamento dos músculos acessórios da inspiração. Os dados coletados foram as medidas de pressões inspiratórias e expiratórias máximas (PIM e PEM, capacidade inspiratória (CI, pico de fluxo expiratório (PFE, mobilidade torácica por meio do coeficiente respiratório (Cr nos níveis axilar (CrA, xifoide (CrX e umbilical (CrU e análise biofotogramétrica do ângulo de Charpy. Estes foram verificados antes e após 10 e 20 atendimentos.     Full Text Available Hanseníase é uma doença causada pelo Mycobacterium leprae que afeta principalmente pele e nervos. Em muitos países subdesenvolvidos, a prevalência de HIV e Hanseníase tem aumentado, consequentemente o número de pessoas coinfectadas também. Relata-se o caso de uma paciente coinfectada por essas duas doenças apresentando quadro de úlceras profundas e muito dolorosas em antebraço direito 3 meses após a introdução de terapia antirretroviral. Após investigação para descartar outras causas de lesões cutâneas ulcerativas, a paciente obteve diagnóstico de reação reversa atípica fazendo parte de um quadro de síndrome de reconstituição imune. A paciente recebeu tratamento com prednisona e obteve resolução do quadro. Apesar da infecção pelo vírus HIV ter baixo impacto no desenvolvimento e evolução da Hanseníase, essa micobacteriose, bem como os quadros reacionais associados a ela, podem se apresentar como manifestação de síndrome de reconstituição imune.

    Full Text Available As manifestações gastrointestinais de infeções sexualmente transmissíveis podem não ser precocemente reconhecidas. Perante o aumento dos comportamentos sexuais de risco é imperativo o reconhecimento da proctite infeciosa na prática clínica. Os autores descrevem o caso clínico de um homem de 28 anos, homossexual, com infeção pelo vírus de imunodeficiência adquirida (com contagem de células CD4+ normal, que recorreu por múltiplas vezes ao serviço de urgência por quadro de retorragias e fezes com muco purulento, obstipação com tenesmo, aerocolia intensa e emagrecimento, sem melhoria com tratamento sintomático. A retosigmoidoscopia revelou uma proctite ulcerativa severa, com achados inespecíficos na histologia. A pesquisa de DNA do vírus Epstein-Barr e citomegalovírus em biópsia subsequente foi positiva, pelo que iniciou terapêutica dirigida, com melhoria paulatina do quadro clínico e endoscópico. Este caso realça a complexidade do diagnóstico da proctite infeciosa e alerta para a possível coexistência de múltiplos agentes transmitidos sexualmente.  Recebido: 21/05/2016 - Aceite: 05/06/2016

    ú. Se incluyeron 71 pacientes [49/71 (69% del sexo masculino], con edad media de 37 años. Los diagnósticos fueron clasificados como definitivos (44% o probables (56%. La ocupación más frecuente fue la agricultura (39%. La fuente de infección fue identificada en 63 (88.7% pacientes. Todos presentaron lesiones ulcerativas con necrosis central. La mayoría de ellos (65% tuvieron lesiones múltiples, principalmente localizadas en miembros superiores (80%. Cuatro pacientes (5.6% desarrollaron meningoencefalitis y tres de ellos fallecieron. En conclusión, considerando sus particulares características clínicas y epidemiológicas, el ántrax cutáneo debe ser siempre incluido en el diagnóstico diferencial de las lesiones cutáneas ulcerativas. Los pacientes con sospecha clínica de la enfermedad deben recibir tratamiento precoz con el objetivo de evitar complicaciones neurológicas, las cuales presentan elevados índices de fatalidad.

    Full Text Available Embora a colite ulcerativa seja conhecida desde 1875 e muitas sejam as manifestações extra-intestinais descritas nesta doença, só recentemente chamou-se a atenção para o envolvimento do aparelho respiratório. Severas complicações têm sido descritas em pacientes como: estenose traqueal inflamatória, bronquiolite com pneumonia, pneumonite intersticial, granulomatose de Wegener, bronquite crônica com bronquiectasia, nódulos necrobióticos, vasculites, fibrose e alveolites. O presente estudo visa reconhecer as alterações pulmonares, na fase aguda da doença inflamatória do cólon, induzida em ratos com ácido acético à 10% e comparar com controles normais. Foi possível constatar que 100% dos animais com colite apresentaram reação inflamatória pulmonar (p=0,0210 de intensidade moderada à severa (p=0,0340. Vasculite foi vista em 58,33% dos pulmões (p=0,0060 e em 3 animais detectou-se hemorragia focal, necrose e abscesso. Estes achados permitem atribuir uma forte associação entre a doença inflamatória do cólon e alterações do aparêlho respiratório, durante a fase aguda da doença, em ratos.

    Full Text Available Descreve-se um surto de fotossensibilização causada por favas de Enterolobium contortisiliquum em bovinos no Estado de Pernambuco, Nordeste do Brasil. Os sinais clínicos observados foram hiporexia, prostração, perda de peso, edema generalizado, dermatite e icterícia. Havia anemia e atividades séricas de AST, LDH e GGT elevadas. A concentração sérica de ureia encontrava-se levemente aumentada e creatinina, proteínas séricas e albumina estavam em níveis normais. Na necropsia, o fígado estava aumentado de tamanho e difusamente alaranjado e os rins estavam aumentados de volume e apresentavam estriações esbranquiçadas irregulares entremeadas por áreas escuras na superfície subcapsular. Histologicamente, observou-se na pele, dermatite ulcerativa. No fígado havia vacuolização e necrose de hepatócitos da região centrolobular e ocasionalmente necrose individual de hepatócitos em outras regiões. O rim apresentava nefrose tubular tóxica. O diagnóstico diferencial da intoxicação por E. contortisiliquum na região semiárida deve incluir as intoxicações por Brachiaria spp, Panicum dichotomiflorum e Froelichia humboldtiana, que também causam fotossensibilização.

    The distribution patterns of the erythropoietic marrows of 17 patients with polycythemia vera were evaluated with a Ring-type Whole Body Linear Scanning using 59 Fe. Ferrokinetic indices calculated by Huff's method were as follows: PID T1/2 42 +- 24 min. (Mean +- S.D.), RCU 86 +- 17%, PIT 1.00 +- 0.58 mg/kg/day, EIT 0.91 +- 0.61 mg/kg/day. The TCU and EIT were lower in polycythemia vera cases with myelofibrotic change than in polycythemia vera cases without myelofibrotic change. After the injection of 59 Fe, longitudinal as well as transverse linear scanning were performed at 6 hours and 24 hours, and on the 5th day and 10th day. The distribution patterns of 59 Fe at 24 hours, which reflect the erythropoietic marrow distribution, were divided into 3 patterns: normal, marrow expansion and extramedullary erythropoiesis. Among 20 cases, 5 were normal, 7 had marrow expansion, and 13 had extramedullary erythropoiesis. The frequency of extramedullary erythropoiesis was significantly high in patients with splenomegaly. An accumulation of 59 Fe in the spleen on the 10th day, reflecting hemolysis and pooling of the red cells, was found in 19 of 20 cases polycythemia vera. Whole body linear scanning is useful for the diagnosis, treatment and studies of pathophysiology of polycythemia vera. (J.P.N.)

    Full Text Available Las enfermedades del tubo gastrointestinal forman parte importante de la morbilidad y de la mortalidad mundiales; incluyen desde las enfermedades infecciosas, alérgicas, inflamatorias crónicas (como la enfermedad de Crohn y la colitis ulcerativa hasta padecimientos considerados como funcionales, como el síndrome de intestino irritable. En algunas de ellas el tratamiento puede ser difícil, ya que en ocasiones pueden existir diferentes teorías en relación a las mismas y en las que se han encontrado múltiples factores tanto psicosociales como genéticos. En los últimos años ha cobrado gran importancia el papel que desempeña la microbiota intestinal en la génesis de algunas de ellas.   Desde el siglo XV se describió la presencia de microorganismos en el tubo gastrointestinal, pero fue hasta que se originó la biología molecular cuando se logró un mejor conocimiento de la función y la composición del ecosistema gastrointestinal, el cual contribuye de manera importante para los procesos de digestión y absorción de sustratos de la dieta, así como para las funciones inmunológicas y protectoras de la microbiota gastrointestinal en cada organismo, lo que resulta esencial para comprender su participación en el tratamiento y en la prevención de múltiples enfermedades.

    Full Text Available Background: Ulcerative colitis patients who underwent restorative proctocolectomy with ileal pouch-anal anastomosis can develop various mechanical complications. Among them is presacral sinus resulting from chronic anastomotic leak. Methods: We present a symptomatic patient with a large fecalith blocking the sinus which was successfully treated with Doppler ultrasound guided endoscopic needle knife sinusotomy along with fecalith extraction. Results: A 67-year-old female presented with a 4-month history of perianal pain and urgency. Pouchocopy showed a 3-cm deep wide-mouthed anastomotic sinus, the orifice of which was blocked by a large hard fecalith. Removal of the fecalith using RothNet, Tripod, or Basket were made but failed. Then needle knife was applied to cut the orifice to enlarge the opening of the sinus. One month later, the patient returned and the fecalith was successfully removed with two Baskets and two Rothnets. Six months after fecalith extraction, pouchoscopy showed a compartalized distal pouch sinus with two cavities, which was treated by two sessions of Doppler ultrasound guided endoscopic needle knife sinusotomy. Six months following the treatment, the sinus was completed healed. The patient tolerated all procedures well without any complication. Conclusion: Fecalith blocking pouch anastomotic sinus is a rare complication in patients with restorative proctocolectomy. In our case, this surgical complication was successfully treated with a carefully planned, stepwise endoscopy approach.     Full Text Available Analysis of descriptions of the “Cookie Theft” picture from the Boston Diagnostic Aphasia Examination have been shown to (1 distinguish between controls, and chronic mild, moderate, and severe aphasia(Craig et al., 1993; Yorkston & Beukelman, 1980; and (2 distinguish distinct profiles of deficits in chronic right hemisphere (RH stroke (Myers, 1978; Trupe & Hillis, 1985. We hypothesized that analysis of the “Cookie Theft” picture descriptions in acute stroke would also: (1 provide quantitative measures of severity of communication impairment that correlate with volume of infarct or volume of lesion in key regions of interest; and (2 provide quantitative measures of hemispatial attention that distinguish RH from left hemisphere (LH stroke and from controls. Methods: We analyzed picture descriptions of patients with acute ischemic LH (n=28, RH (n=25 stroke, and healthy controls (n=25 for total content units (CU produced by previously studied healthy controls during picture descriptions (Yorkston & Beukelman, 1980, syllables/CU, and ratio of left page:right page CU (LCU:RCU. CUs defined as nouns, adjective, and verbs relevant to the picture. Diffusion-weighted images were registered to a common atlas, to measure volume of infarct and percent damage to regions of interest without knowledge of behavioral analysis. We evaluated differences across groups with ANOVA and across pairs of groups with t-tests. Dentre as doenças vesiculares ou erosivas, descrevem-se os principais aspectos relacionados ao diagnóstico da estomatite vesicular, diarréia viral bovina, febre catarral maligna, infecções por herpesvírus bovino 1 e 5, e uma estomatite ulcerativa associada a parvovírus bovino, que ocorreu no Rio Grande do Sul; língua azul, para a qual tem sido detectados anticorpos em bovinos e ovinos do Rio Grande do Sul; mamilite herpética que ocorre em outros Estados do País;peste bovina, que foi diagnosticada e erradicada no Estado de São Paulo em 1921; estomatite popular; e duas doenças exóticas:exantema vesicular e doença vesicular do suíno.Diseases to be considered in the differential diagnosis of foot-and-mouth disease are reviewed. The main aspects relating to the diagnosis of vesicular stomatitis, bovine virus diarrhea, malignant catarrhal fever, bovine herpesvirus 1 and 5, andem ulcerative stomatitis associated with bovine parvovirus are described. Bluetongue, that probably occurs in Rio Grande do Sul because antibodies to the virus have been detected in cattle and sheep; is refered. Bovine ulcerative mammilitis, reported in other Brazilian States, rinderpest, reported and eradicated in the State of São Paulo in 1921, and popular stomatitis are also cited, and so are two exotic diseases: vesicular exanthema and swine vesicular disease.

    Full Text Available Foram realizadas pesquisas sobre a natureza e causa de lesões de pele em equídeos em uma propriedade no município de Castanhal, região Nordeste do Estado do Pará. Foram realizadas visitas técnicas, estudos epidemiológicos, coletas de sangue, biópsias de pele afetada e a inspeção da pastagem. O estudo incluiu 25 equídeos, dos quais 14 machos e 11 fêmeas, de seis meses e oito anos de idade. Os animais apresentaram lesões ulcerativas, de bordos irregulares, na cabeça (narinas, focinho, lábios superiores e inferiores e chanfro, na cavidade oral (vestíbulo bucal e gengiva e nos membros (boletos, metacarpos e metatarsos e articulação escápulo-umeral. No exame histopatológico foram observados focos de erosões cutâneas, caracterizados por perda e necrose da epiderme, com espongiose, degeneração vesicular da epiderme remanescente e leve infiltrado inflamatório na derme subjacente, constituído predominantemente por macrófagos e, em menor grau, eosinófilos. Na inspeção da pastagem, constituída de Brachiaria humidicola, foi constatada grande invasão de duas plantas providas de espinhos, Mimosa pudica e Mimosa debilis, ambas da família Leg. Mimosoideae. Concluiu-se, que as lesões de pele foram causadas pela ação traumática dos espinhos de Mimosa pudica e Mimosa debilis.

    Full Text Available Paracoccidiodiomicosis (PCM es una micosis sistémica granulomatosa causada por el hongo Paracoccidioides brasiliensis. Basado en la sintomatología clínica, radiológica e histológica debe de considerarse como una patología diferencial del carcinoma de células escamosas, tuberculosis, así también como enfermedad oportunista, en pacientes con alteración del sistema inmune. Se reporta un caso de paracoccidioidomicosis en un paciente masculino, 62 años de edad, procedente, Alauca (Honduras, que se dedica a la agricultura que refiere disfagia de 6 meses de evolución acompañándose de otalgia del lado derecha, un mes de pérdida de peso y tos con esputo hemoptoico. Al examen físico mucosas húmedas con abundante sialorrea y lesión ulcerativa en amígdala derecha, adenopatía cervical en región lateral derecha, doloroso, con cambios inflamatorios. El diagnóstico se realizó por la biopsia de amigdala que se reportó como células con múltiples brotes forma de "timón de barco" compatibles con P. brasiliensis. Típicamente esta enfermedad tiene evolución subaguda o crónica y normalmente se reciben tratamiento para otras patologías antes de poder lograrse un diagnostico definitivo. En personas de zonas endémicas que presenten factores de riesgo, se debe de realizar los estudios para descartar dicha micosis permitiendo un diagnóstico precoz.

    The ALICE HLT uses a data transport framework based on the publisher- subscriber message principle, which transparently handles the communication between processing components over the network and between processing components on the same node via shared memory with a zero copy approach. We present an analysis of the performance in terms of maximum achievable data rates and event rates as well as processing capabilities during Run 1 and Run 2. Based on this analysis, we present new optimizations we have developed for ALICE in Run 2. These include support for asynchronous transport via Zero-MQ which enables loops in the reconstruction chain graph and which is used to ship QA histograms to DQM. We have added asynchronous processing capabilities in order to support long-running tasks besides the event-synchronous reconstruction tasks in normal HLT operation. These asynchronous components run in an isolated process such that the HLT as a whole is resilient even to fatal errors in these asynchronous components. In this way, we can ensure that new developments cannot break data taking. On top of that, we have tuned the processing chain to cope with the higher event and data rates expected from the new TPC readout electronics (RCU2) and we have improved the configuration procedure and the startup time in order to increase the time where ALICE can take physics data. We analyze the maximum achievable data processing rates taking into account processing capabilities of CPUs and GPUs, buffer sizes, network bandwidth, the incoming links from the detectors, and the outgoing links to data acquisition.

    Obiettivo: nel quadro delle infrequenti manifestazioni extraintestinali dell’amebiasi, le localizzazioni cutanee relative, in corso della mutevole reattività ospite parassita, risultano molto rare. È esposto, pertanto, un caso di amebiasi cutanea, caratterizzato dalla presenza del parassita nelle lesioni dermiche, con i suoi particolari aspetti immuno istologici e clinici, che ne derivano.

    Materiali e metodi: uomo di aa. 30, iraniano, residente in Messina, da più tempo. Nel corso di screening parassitologico presso il Centro Assistenza Immigrati, è risultato portatore (fecale di cisti di E. Histolytica. Durante la permanenza in Iran, lavoro agricolo con maneggio di fertilizzanti fecali; abuso di alcolici da più anni. Epatomegalia; in sede mesogastrica, presenza di una lesione ulcerativa, rotondeggiante (cm 2,5 D con induito rossastro e margini sclerotici e dolenti. Esame istologico da frammento bioptico: infiltrato monocitario con degenerazione degli elementi cutanei, e presenza nell’essudato di trofozoiti di E. Histolytica. Parametri immunoematologici: corrispondenti a paziente alcolista, con immunodeficienza cellulo mediata, e negatività per HIV.

    Risultati: adoperato trattamento con metronidazolo e paramomicina, insieme a terapia correttiva dei disordini alcolcorrelati, nonché dell’immuno deficit mediato. Aggiunto, inoltre, trattamento topico con pasta di RAVAULT’S, medicata con idroemetina, a forti dosi. Guarigione senza recidive, dopo 4 settimane del complessivo trattamento.

    Conclusioni: facilitazione impianto e/o colonizzazione E. Histolytica in sede cutanea, in rapporto ad alterazioni conseguenti ad abuso alcolico e/a immunodeficit cellulo mediato, svolgentisi da più anni, nel caso descritto. Tale lesione cutanea, osservata in soggetto alcolista e immunocompromesso, può rientrare, certamente, nel quadro delle infezioni opportunistiche.

    Full Text Available La cistitis intersticial se define como un síndrome clínico caracterizado por un incremento de la frecuencia urinaria, urgencia miccional y/o dolor abdominal o perineal en ausencia de infección urinaria o enfermedad conocida del aparato urinario. Se divide de acuerdo a los hallazgos citoscópicos en ulcerativa o no. La base del diagnóstico es clínica apoyada en scores de probabilidad y pruebas invasivas. Con la comprensión de la fisiopatología se ha desarrollado un abanico de posibilidades terapéuticas. Comunicamos el caso de una paciente sintomática por cuatro años con diagnóstico de cistitis intersticial con úlcera de Hunner por cistoscopia y excelente respuesta a la amitriptilina. Es una enfermedad que produce considerable alteración de la calidad de vida, por lo que es importante que los médicos estén familiarizados con este cuadro.Interstitial cystitis is characterized by over 6 months of chronic pain, pressure and discomfort felt in the lower pelvis or bladder. It is often relieved with voiding, along with daytime frequency and nocturia in the absence of an urinary tract infection. The disorder can be divided clinically into two groups -ulcerative and non-ulcerative- based on cystoscopic findings and response to treatment. Management follows an approach of applying the least invasive therapy that affords sufficient relief of symptoms. We report a case of a patient with interstitial cystitis. The diagnosis was performed by symptoms and lesion in the cystoscopy and excellent response to amitriptyline.

    Full Text Available La enfermedad inflamatoria intestinal (EII se caracteriza por la activación inapropiada del sistema inmune de la mucosa intestinal y sus dos formas de presentación son: la colitis ulcerativa y la enfermedad de Crohn. Las manifestaciones extraintestinales se presentan hasta en el 36% de los pacientes y pueden comprometer cualquier órgano o sistema. La disfunción inmune se caracteriza por el desequilibrio entre los mediadores proinflamatorios y los antinflamatorios y se expresa como una enfermedad sistémica. Las manifestaciones reumatológicas asociadas a la EII son de tres tipos: la artritis periférica, la espondiloartropatía y una tercera categoría que incluye lesiones dérmicas, oftálmicas y del metabolismo óseo, entre otras. El manejo de estas manifestaciones se basa en la terapia sistémica para el control de la actividad inflamatoria local utilizando esteroides, derivados de la 5-ASA, inmunomoduladores y, en los últimos años, terapia anti-TNF. The main feature of inflammatory bowel disease (IBD is the continuous activation of the mucosa-associated immune system; the disease has two major forms of presentation: ulcerative colitis and Crohn´s disease. The extraintestinal manifestations are present in 36% of patients, and any organ can be affected. There is an imbalance between proinflammatory and antinflammatory cytokines leading to a systemic disease. The rheumatologic manifestations of the IBD are: Peripheral arthritis, spondyloarthropathy and a third category that includes dermic and ocular lesions as well as metabolic bone disease. Control of the extraintestinal manifestations is based on systemic therapy with steroids, 5-ASA derivatives and biological anti-TNF therapy.

    Recent studies have demonstrated the benefits of immunomodulators, such as arginine, in the regulation of inflammatory responses and trophism of the intestinal mucosa. The aim of this study was to evaluate the possible mechanisms action of arginine (pretreatment or treatment) in experimental model of ulcerative colitis induced by dextran sodium sulfate (DSS). C57BL/6 mice were randomized into 5 groups: Control group (C): standard diet and water; Arginine group (ARG): diet supplementation with arginine and water; Colitis group (COL): standard diet and DSS solution; Pretreated group (PT): diet supplementation with arginine before and during colitis induction; Treated group (T): diet supplementation with arginine during colitis induction. Colitis was induced by administration of 1.5% DSS for 5 days. After this, all the mice were euthanized and blood, organs and intestinal fluid were collected for carrying out analyzes. Parameters such as intestinal permeability (IP), bacterial translocation (BT), histological analysis (histological score, morphometric analysis, collagen and mucins stain), nitrate and nitrite, cytokines and chemokines, secretory immunoglobulin A (sIgA), inflammatory infiltrate and oxidative stress were performed. The ARG group did not show difference compared to group C in the investigated parameters (C vs ARG: p> 0.05). The COL group showed increased IP (C vs COL: p < 0.05) and BT (C vs COL: p <0.05). In the histological analysis, the COL group showed severe inflammation and reduction the crypts length. In addition, in the group COL observed increase infiltration of eosinophils, neutrophils and macrophages in the colon, increase cytokine IL-17 and chemokine KC in serum and oxidative stress in the colon (COL vs C: p <0.05). In the arginine-supplemented groups (PT and T) was observed decrease IP and BT to blood, liver and lung (PT and T vs Col: p <0.05). Histological analysis showed that the arginine (PT and T) preserved the intestinal mucosa and crypts length. Furthermore, arginine (PT and T) reduced the infiltration of eosinophils and macrophages and oxidative stress in the colon (PT and T vs. COL: p <0.05). Only treatment with arginine (group T) increased collagen area and nitrite/nitrate concentration in the colon (T and C vs. COL: p <0.05), reduced the cytokine IL-17 in serum (T vs. COL: p <0.05) and increased cytokine TGF-β in serum and colon (T vs C: p < 0.05). In conclusion, the study showed that arginine restored the intestinal epithelium and decreased inflammation. Treatment with arginine (group T) increased collagen e NO production. Furthermore, modulated the immune response increasing the concentration of TGF-β cytokine and reducing IL-17 cytokine. These results suggest that this amino acid can constitute in potential therapy for intestinal inflammatory diseases. (author)

  15. Evaluación clínica e histopatológica de la Pythiosis cutánea en terneros del departamento de Córdoba, Colombia

    Full Text Available Objetivo. Evaluar y caracterizar los aspectos clínicos e histopatológicos de la Pythiosis cutánea en terneros del departamento de Córdoba, Colombia. Materiales y métodos. Se realizó un estudio de tipo descriptivo, no probabilístico en animales por conveniencia. Fueron analizados 14 terneros con Pythiosis cutánea, a los cuales se les realizó caracterización clínica e histopatológica mediante tinciones de Hematoxilina-Eosina y Grocott. Resultados. Clínicamente, los animales presentaron lesiones cutáneas ulcerativas y húmedas con abundante exudación fibrino sanguinolenta, algunas tenían superficie irregular con nodulaciones y hemorragias, así como salida en algunos casos de material caseificado blanco amarillento. Las lesiones se ubicaron en la mayoría de los casos a nivel de las partes distales de los miembros y región ventro-lateral del cuello. Histopatológicamente, en coloración de Hematoxilina-Eosina se observó múltiples piogranulomas constituidos por área central con aglomerados de eosinófilos y pocos neutrófilos rodeados por macrófagos y células gigantes (reacción de Splendore-Hoeppli. En la coloración de Grocott se evidenciaron múltiples hifas intralesionales septadas y parcialmente ramificadas en ángulo recto de color café. Conclusiones. El diagnóstico definitivo de la enfermedad se basó en la epidemiología, características macroscópicas de las lesiones y en los hallazgos histopatológicos, siendo concluyente como métodos de diagnóstico de la Pythiosis cutánea bovina.

    Full Text Available INTRODUÇÃO: A maioria das mulheres que desenvolvem doença inflamatória intestinal (DII encontra-se em idade fértil, despertando preocupação dos médicos e mulheres no entendimento desta interação. Avaliamos a influência da DII sobre a fecundidade e gestação e vice- versa. MÉTODOS: Os protocolos de pacientes com doença de Crohn (DC e retocolite ulcerativa (RC, de 1984 a 2006, em idade fértil, cadastrados no ambulatório de DII, foram revisados. Pacientes foram entrevistados para preenchimento de dados não encontrados nos protocolos. Outros tipos de colites, investigação incompleta, pacientes que não estavam em idade fértil ou sem capacidade cognitiva foram excluídos. Prematuridade, baixo peso ao nascer, anomalias congênitas, natimortalidade, abortamentos, tipos de partos, localização da doença na gestante e uso de medicamentos durante a gestação foram investigados. O método estatístico adotado foi o teste de qui- quadrado e Fisher, com nível de significância de 5%. Nenhum paciente se recusou a participar desta pesquisa. RESULTADOS: 140 gestações em 104 pacientes com DII foram avaliados (RC em 63 gestações e DC em 77. Houve redução da fecundidade após o início dos sintomas relacionados à DII em 41,6%, com influência da doença sobre a opção de não ter filhos em 20,6% (10,3% dos pacientes por medo da doença; 6,5% por orientação médica e 2,2% por más condições clínicas, sem diferenças entre DC e RC. A grande maioria não quis engravidar por já ter filhos, por ser solteira ou estar ter baixa idade (53,3%. A atividade da RC não foi alterada durante a gestação na maioria das pacientes (77,8%; p>0,003. A atividade da doença melhorou durante a gestação mais nas gestantes com DC do que nas com RC (p>0.0007.     Full Text Available Descreve-se a ocorrência de pitiose cutânea em bovinos de corte na região sul do Rio Grande do Sul. Os animais foram introduzidos em área alagada e desenvolveram lesões cutâneas ulcerativas e úmidas, de tamanhos variados, localizadas na região distal dos membros e no chanfro nasal. Histologicamente, as lesões eram caracterizadas por múltiplos granulomas com hifas intralesionais, melhor observadas pela coloração de metenamina nitrato de prata de Gomori, e circundadas por abundante tecido conjuntivo fibroso. O diagnóstico foi realizado com base na epidemiologia, lesões macroscópicas e histológicas e pela reação imuno-histoquímica positiva com anticorpo policlonal anti-Pythium insidiosum. A morbidade foi de 23,8% e os animais adoeceram 15-90 dias após a introdução na área alagada. Em todos os casos as lesões evoluíram para a cura sem tratamento. Sugere-se que a doença possa ser mais frequente em bovinos do que se supõe, principalmente em áreas alagadas da região Sul do Rio Grande do Sul.An outbreak of cutaneous pythiosis is described in cattle from southern Rio Grande do Sul, Brazil. Cattle were introduced into flooded pastures and developed ulcerative and wet cutaneous lesions on distal limbs and Planum nasale. Histologically, the lesions were characterized by multiple granulomas with intralesional hyphae, better seen in the methenamine silver stain, and surrounded by abundant fibrous tissue. Diagnosis was based on epidemiology, gross and histological lesions, and by positive immuno-histochemical reaction with anti-Pythium insidiosum polyclonal antibody. Morbidity was 23.8% and lesions were observed 15-90 days after cattle were introduced in the flooded area. Affected cattle had spontaneous healing without treatment. It is suggested that the disease is more frequent than what was earlier thought, mainly in flooded areas of the Southern region of the Rio Grande do Sul State.

  18. Exposición espontánea de un torus palatino de la línea media

    Full Text Available Introducción. El torus, o rodete, palatino es una alteración ósea benigna que puede generar molestias en la fonación y en la deglución. Cuando su crecimiento o exposición persistente producen síntomas molestos, debe ser resecado quirúrgicamente. Presentación del caso. Se trata de una paciente de 82 años de edad, que consultó por una lesión ulcerativa de un año de evolución, que producía dolor, halitosis y molestia a la deglución. En el examen físico de la cavidad oral, se observó una prominencia de consistencia dura en la línea media, con perforación y ulceración de la mucosa palatina en su porción lateral posterior izquierda. Se consideraron los diagnósticos de torus palatino, osteoma, granuloma piógeno y neoplasia de tejidos blandos. Se decidió resecarla quirúrgicamente y hacer el estudio histopatológico. La evolución clínica fue satisfactoria, con resolución completa un mes después de la cirugía. En el estudio de histopatología se observó hiperostosis ósea con inflamación crónica de los tejidos blandos e hiperplasia simple de la mucosa, que corresponden a un torus palatino. Discusión. Se reporta un inusual caso de exposición espontánea de un torus palatino que ocupaba casi toda la bóveda palatina, se describe su resección quirúrgica y se informan los hallazgos de histopatología.     New compounds of the series CaCu{sub 3}(Mn{sub 4−x}Al{sub x})O{sub 12} have been prepared under high pressure conditions (2 GPa), in the presence of KClO{sub 4} as oxidizing agent to stabilize Mn{sup 3+,4+} mixed valence. The polycrystalline samples have been characterized by x-ray diffraction, neutron powder diffraction (NPD), magnetic, and magnetotransport measurements. All the samples are cubic, space group Im-3. These oxides adopt a superstructure of ABO{sub 3} perovskite given by the long-range 1:3 ordering of Ca{sup 2+} and Cu{sup 2+} ions at the A sublattice. The NPD study for x = 0.4 shows that Al{sup 3+} ions are statistically distributed at the octahedral positions, being the (Mn,Al)O{sub 6} octahedra strongly tilted, with superexchange (Mn,Al)-O-(Mn,Al) angles of 142.1°. Also, neutron data clearly show that some Mn{sup 3+} ions (0.65(2) per formula) are located together with Cu{sup 2+} at the square-planar 6b positions. Regarding the magnetic properties, all the compounds present a spontaneous increase of the magnetization below T{sub C}, typical of ferro-or ferrimagnetic materials, with T{sub C} decreasing upon Al introduction. The magnetic structure determined from low-temperature NPD data unveils a ferromagnetic coupling between (Cu{sup 2+}, Mn{sup 3+}){sub 6b} spins and Mn{sub 8c} spins at octahedral positions; this is in contrast with the ferrimagnetic structure observed for RCu{sub 3}Mn{sub 4}O{sub 12} and CaCu{sub 3}Mn{sub 4}O{sub 12}, where an AFM coupling is observed between both magnetic sublattices. Interestingly, an enhancement of the magnetoresistance effect is observed for x = 0.2, well beyond that found for the parent compound. This effect, in materials subtly doped with non-magnetic elements at the Mn positions, may be of interest for applications.

  20. Indagine sulle malattie infiammatorie croniche intestinali in un'azienda ULSS veneta

    Obiettivi: l’epidemiologia delle malattie infiammatorie croniche intestinali fino ad adesso in Italia è stata poco studiata. Da alcuni studi emerge una minor incidenza di questi fenomeni nel Sud Europa rispetto al Nord Europa e agli Stati Uniti. Lo scopo di questa indagine è quello di valutare l’incidenza della Rettocolite Ulcerosa (RU e del Morbo di Crohn (MC nella popolazione dell’Azienda ULSS 17, situata nella provincia di Padova.

    Metodi: per la realizzazione di questa indagine ci si è avvalsi della collaborazione di 23 Medici di Medicina Generale (MMG operanti in 14 comuni dell’ULSS 17. I dati sono stati prelevati dal sistema informatizzato comune ai 23 MMG (Millenium, in base ai codici 555 e 556 dell’ICD9. È stato considerato il periodo di tempo 1995 - 2001. La popolazione media di riferimento è di 29.740 abitanti. Risultati: nel periodo considerato sono stati registrati in totale 45 nuovi casi, 35 casi di RU e 10 di MC. L’incidenza media della RU è stata di 16,76/100.000, quella del MC di 4,7/100.000. La RU ha avuto un’incidenza maggiore nel sesso maschile (rapporto 2:1, mentre per il MC si rileva l’opposto (rapporto 2:3. L’età media alla diagnosi per la RU è stata di 44 anni, quella del MC di 51,9. Dall’analisi temporale si evidenzia un progressivo aumento dell’incidenza del MC, mentre la RCU è più costante.

    Conclusioni: paragonando i risultati ottenuti con quelli di altre indagini condotte in Italia, l’incidenza delle malattie infiammatorie croniche intestinali è apparentemente più elevata nel territorio dell’ULSS 17. Dalla revisione della letteratura nazionale e internazionale emerge che variazioni dell’incidenza di questi fenomeni possono essere ricondotte ad un diverso accesso ai servizi sanitari, a diverse pratiche diagnostiche o alla presenza di fattori di rischio di tipo genetico o ambientale

    Full Text Available La estrongiloidiasis diseminada (ED es una afección frecuentemente fatal debida a la autoinfección interna con larvas de Strongiloides stercoralis, oportunjsta ante alteraciones de la inmunidad del huésped, como inmunosupresión farmacológica en pacientes transplantados o con enfermedad neoplásica o autoinmune. Revisamos la ED en los archivos de autopsias del Hospital San Juan de Dios y La Misericordia de Bogotá, para conocer la entidad básica facilitadora de la diseminación larvaria y averiguar si la ED ha aumentado recientemente. Se encontraron sólo 12 enfermos en más de 14.000 protocolos revisados desde 1954 a 1986 con edades entre 1 y 62 años. Los pacientes presentaron diarrea crónica, fétida o sanguinolenta, vómito, dolor abdominal, anorexia y tos, a veces hemoptoica. Todos tenían desnutrición avanzada y sólo uno había recibido tratamiento inmunosupresor con esteroides y ciclosfosfamida para glomérulonefritis progresiva. Un paciente recibió esteroides para "colitis ulcerativa" que luego se comprobó era debida a estrongiloidiasis. Sólo una vez se hizo diagnóstico clínico de ED. En tres pacientes se encontraron larvas en la materia fecal pero no se consideró la posibilidad de ED. Los diagnósticos clínicos considerados fueron: desnutrición, síndrome anémico, carcinoma gástrico, TBC pulmonar e intestinal, parasitismo y síndrome meníngeo. Una enferma presentó endocarditis bacteriana tricuspídea y dos meningoencefalitis, unidas al cuadro gastrointestinal. La meningitis fue de tipo bacteriano, aguda, sin granulomas ni larvas. Se encontraron larvas con inflamación severa granulomatosa en intestino delgado, colon, pulmón, hígado y ganglios linfáticos. En cuatro pacientes fue llamativo el hallazgo de trombosis de grandes venas: cava, femoral, mesentérica y de seno longitudinal superior. La ED si bien no es frecuente y no ha aumentado entre nosotros como consecuencia de inmunosupresión, debe tenerse en cuenta

    Full Text Available O Herpes simplex (HSV é um DNA vírus que provoca afecções perianais, sendo considerada a causa mais comum das úlceras na região. Apesar da forma ulcerativa ser a mais conhecida, a literatura relata o aparecimento de lesões tumorais, nodulares ou hipertróficas relacionadas ao vírus. O exame proctológico mostra tumores dolorosos, achatados, com superfície recoberta por ulceração rasa e com bordas bem delimitadas, elevadas e lobuladas, localizados na margem anal e/ou no sulco interglúteo, algumas vezes imitando condilomas virais ou carcinoma. A anamnese revela instalação insidiosa com crescimento lento e progressivo, além da história de tratamentos anteriores para úlceras herpéticas. O diagnóstico diferencial com carcinoma impõe a realização de biópsia para confirmação histológica. Esse exame revela hiperplasia epitelial moderada e denso processo inflamatório com linfócitos e plasmócitos. Células gigantes e multinucleadas são observadas na epiderme. Os testes imunohistoquímicos sugerem o HSV. A opção terapêutica inicial deve ser o tratamento medicamentoso. Importante definir o diagnóstico etiológico para aliviar o desconforto e evitar operação radical desnecessária, e introduzir medicação anti-retroviral nos portadores do HIV para melhora da imunidade.Herpes simplex is a DNA virus which provokes perianal lesions, and it is the most frequent etiology of anal ulcer.     Full Text Available OBJETIVO: Estimar a prevalência de manifestações oculares na população local de artrite reumatóide (AR. Estimar se a presença de auto-anticorpos como fator reumatóide (FR e fator antinuclear (FAN influi no aparecimento destas manifestações e se existe associação entre o aparecimento de manifestações oculares e índice funcional do paciente, idade ao diagnóstico e tempo de doença. MÉTODOS: Foram estudados retrospectivamente 198 prontuários de pacientes com AR, acompanhados durante o período de 2003 a 2006. Avaliou-se a prevalência das manifestações oftalmológicas, perfil de auto-anticorpos, dados demográficos e índice funcional de Steinbrock destes pacientes. RESULTADOS: Síndrome de Sjögren secundária apareceu em 12,1% dos pacientes sendo mais comum em mulheres (p=0,049 e em pacientes com maior dano articular (p=0,016. Ceratite ulcerativa e esclerite incidiram em torno de 2% dos pacientes. Fator reumatóide (FR e fator antinuclear (FAN assim como tempo de doença e idade do paciente ao diagnóstico não influíram no aparecimento das manifestações estudadas.     Full Text Available Introduction. Maggot debridement therapy has been widely used for treating a variety of scarred-over soft-tissue wounds. Published accounts record several illnesses in which treatment with larval therapy has promoted injury healing in conjunction with infection by bacterial pathogens resistant to conventional antibiotics.
    Objective. An initial test of the maggot therapy was developed for cutaneous injuries produced by Leishmania amazonensis.
    Materials and methods. An experimental design based on an animal model with three replicates in Mesocricetus aureatus (Rodentia: Muridae was used to evaluate size variation lesion before and after after larval therapy with Lucilia sericata maggots. The criteria used for therapy evaluation were lesion size, maggot application time, and presence or absence of edema and secretions.
    Results.  Effective scarring and wound healing was observed after therapy with L. sericata larvae, i.e. 80% to 100% lesion area reduction after 12 hours.
    Conclusion. The preliminary results suggest that fly maggots of L. sericata have a potential use as natural medical and veterinary alternative therapy for the cutaneous leishmaniasis.

    Full text: The policy established by the National Government of Argentina in August 2006 related to resuming the Nuclear activity in the country, lead the CNEA trough the Exploration of Raw Materials Manager (ERMM) to establish working strategies for the next 10 years. These strategies together with the assignment of an adequate budget will contribute to define new uranium resources, which together with the already known ones, will be used to supply the requirements of Nuclear Power and Research Plants in the future. Thus, the ERMM is applying a policy of human resources hiring new personnel in order to count with the minimum necessary workforce to reach these tasks. In Argentina known U resources are related to sedimentary, igneous and metamorphic environments. Considering the geology of the different regions, Argentina has been divided into 57 units in which the geological, geochemical, mineralogical and structural information is evaluated in order to estimate the uranium geological favorability of each unit. The final pursuit of this regional study is to circumscribe new areas with anomalous uranium contents in which prospection and exploration should be carried out. These studies together with prospection and exploration works are performed in the country by four exploration centers based in Salta (RN), Cordoba (R.Ce), Mendoza (R.Cu) and Trelew (RP). The works planned for each exploration center includes: Regional Noroeste, Mina Franca Deposit: peri-granitic vein- type mineralization: 25% of surface exploration has been performed. Mineralized areas: Istataco and San Buenaventura correspond to an igneous-metamorphic environment, Sierra de Vaqueria to a sedimentary one: Prospection stage.     Full Text Available O aumento da intensidade da quimioterapia e radioterapia no tratamento do câncer tem elevado a incidência de efeitos colaterais, em especial da mucosite bucal. OBJETIVO E MÉTODO: Através de revisão bibliográfica realizou-se atualizar informações quanto à definição, características clínicas, incidência, etiologia, patofisiologia, morbidade associada, prevenção e tratamento dessa manifestação clínica. RESULTADOS: Estudos atuais definem a mucosite bucal como uma inflamação e ulceração dolorosa bastante freqüente na mucosa bucal apresentando formação de pseudomembrana. Sua incidência e severidade são influenciadas por variáveis associadas ao paciente e ao tratamento a que ele é submetido. A mucosite é conseqüência de dois mecanismos maiores: toxicidade direta da terapêutica utilizada sobre a mucosa e mielossupressão gerada pelo tratamento. Sua patofisiologia é composta por quatro fases interdependentes: fase inflamatória/vascular, fase epitelial, fase ulcerativa/bacteriológica e fase de reparação. É considerada fonte potencial de infecções com risco de morte, sendo a principal causa de interrupção de tratamentos antineoplásicos. Algumas intervenções mostraram-se potencialmente efetivas para sua prevenção e tratamento.     Full Text Available Background: Inflammatory polyps are common sequelae in patients with inflammatory bowel diseases (IBD. Those polyps can usually be removed with snare polypectomy. There were limited data evaluating the management of hot snare-resistant inflammatory polyps. Methods: We reported on two cases with hot snare-resistant inflammatory polyps, one was a Crohn's disease (CD patient with the polyp at the ileo-colonic anastomosis (ICA and the other one was an ulcerative colitis (UC patient with polyp at the pouch inlet. Results: Sedated endoscopy was performed, which showed a large 2.5 cm pedunculated polyp at the ICA in the first patient and a large 5 cm pedunculated polyp at the pouch inlet in the second patient. Hot snare polypectomy was initially attempted, but failed in both pa- tients. Then endoscopic needle knife polypectomy was performed, which helped complete polypectomy. Both procedures took approximately 25 minutes each. The patients tolerated the procedure well and continued to do well after the procedure. The final pathological diagnoses for both patients were inflammatory polyps with extensive fibrosis. Conclusions: Endoscopic needle knife-assisted polypectomy appeared to be an effective technique for the management of hot snare-resistant inflammatory polyps.     Full Text Available OBJETIVO: Confrontar com a literatura os dados relacionados à idade, sexo e indicação dos transplantes penetrantes de córnea efetuados no Hospital São Geraldo da Universidade Federal de Minas Gerais (UFMG, de janeiro de 1999 a dezembro de 2005. MÉTODOS: Realizou-se estudo retrospectivo de 887 ceratopatias penetrantes, com dados obtidos dos arquivos da Central Estadual de Notificação de Minas Gerais, MG Transplantes, e dos prontuários dos pacientes do Departamento de Córnea do Hospital São Geraldo. Os dados analisados foram: idade, sexo e diagnóstico pré-operatório das alterações corneanas. As indicações de transplante do período de janeiro de 1999 a junho de 2002 foram comparadas as de julho de 2002 a dezembro de 2005. RESULTADOS: A média da idade dos pacientes foi de 48,3 anos, variando de 4 meses a 97 anos. As principais indicações de transplante penetrante de córnea foram: lesão ulcerativa (34,95%, ceratocone (16,35%, ceratopatia bolhosa do psedofácico (16,01%, leucoma (14,09%, falência primária (7,89%, rejeição (5,64%, distrofia endotelial de Fuchs (1,92%, outras distrofias (1,47%, causa indeterminada (1,23% e outras causas (0,45%. CONCLUSÃO: A principal indicação de transplante foi para úlcera de córnea em vias de perfuração ou perfurada, responsável por 310 ceratopatias penetrantes. As indicações eletivas mais frequentes de transplantes foram para ceratocone, ceratopatia bolhosa do psedofácico e leucoma, com o crescimento das indicações para ceratopatia bolhosa do psedofácico no período.     Full Text Available FUNDAMENTOS: O pioderma gangrenoso é doença neutrofílica pouco freqüente. Caracteriza-se por lesões cutâneas ulceradas, dolorosas, com bordas subminadas e violáceas. Os membros inferiores configuram o local mais acometido. Sua etiologia é incerta, mas em 50% dos casos encontra-se associação com outras doenças. A histopatologia é inespecífica, e o diagnóstico, essencialmente clínico. OBJETIVO: Avaliar o perfil clinicopatológico de pacientes com pioderma gangrenoso. MÉTODO: Estudo retrospectivo dos pacientes diagnosticados no período de 2000 a 2006 no Serviço de Dermatologia do Hospital de Clínicas de Porto Alegre. RESULTADOS: Foram observados 16 pacientes cuja idade média foi 49 anos, com predomínio do sexo feminino (62,5%. O período médio entre início da doença e diagnóstico foi de 1,6 ano. A forma clínica predominante foi a ulcerativa (81,25%, e 87,5% das lesões localizavam-se nos membros inferiores. O sintoma mais freqüentemente associado foi dor local (37,5%. Doze pacientes (66% apresentaram doenças sistêmicas concomitantes. Doença de Crohn, diabetes e colagenoses foram as principais comorbidades encontradas. O tratamento mais utilizado foi a corticoterapia sistêmica, associada ou não a outros medicamentos (50%, tendo 43,75% dos pacientes apresentado recidiva do quadro. CONCLUSÕES: Os dados avaliados condizem com os encontrados na literatura. As doenças associadas mais prevalentes foram ileíte regional, diabetes melitus e afecções do tecido conectivo. O tratamento mais utilizado incluiu corticoterapia sistêmica.     Full Text Available The paleoevolution of the river system in the Moldavian Tableland has undergone at a theoretical level two successive hypothetical directions. The former direction, formulated in the first half of the XXth century, alleges the existence of a river system, with a transverse character, that prolonged to the east the Carpathian river systems. Specialists of this theory, Tufescu V. (1932, M. David (1933, quoted by V. Bacauanu - 1973 and Gheorghe Nastase (1946, quoted by V. Bacauanu - 1973 based their design on the presence of horizons of gravels with Carpathian elements on some interfluves to the east of the Siret valley, and the existence of some saddles, Loznica, Bucecea and Ruginoasa, located on the left side of the same river. The latter direction, diametrically opposite, supports the idea of evolution of the river system consecutively with the Sarmato-Pliocene Sea pulling back to the south. The hypothesis is supported and substantiated by I. Sîrcu (1955, C. Martiniuc and V. Bacauanu (1960, V. Bacauanu (1968, 1973. In this context it is considered that the Prut and Siret rivers carved the oldest valleys in the tableland. The authors explain the erosional genesis of the saddles arguing their inability to create transversal rivers on the basis of continuity upstream and downstream the saddles of terrace levels with relatively high altitudes, which proves the flow continuity on consequent valleys. The research carried out along the Suceava river valley and the immediate interstream area in order to create a scenario of the valley paleoevolution highlighted the presence of crystalline lithotopes in the petrographic structure of the accumulation deposits. The presence of such petrographic elements contradicts the present-day alluvia sources of the rivers that could have transported them:  Suceava or its tributaries Solonet, Ilisasca, Scheia. The rivers spring either from the Carpathian flysch or from the former area of platform sedimentation. We

    Full Text Available OBJETIVO: Avaliar a atividade do hidrolisado das proteínas de soro de leite bovino e uma fração de peptídeos de baixo peso molecular (peso molecular OBJECTIVE: To assess the ability of bovine whey protein hydrolysate and its low molecular weight fraction (molecular weight <1kDa to protect the gastric mucosa of rats against ulcerative process induced by three different agents. METHODS: Adult Wistar rats were subjected to the indomethacin-induced ulcer (30mg/kg body weigh, absolute ethanol (1ml/animal and immobilization and cold stress (4(0C/2h, models. RESULTS: Whey protein hydrolysate was obtained by treatment with pancreatin to a degree of hydrolysis of 20% and fractionated using a tangential flow membrane with a molecular weight cut-off of 1kDa to obtain the fraction containing low molecular weight peptides (<1kDa. In the ethanol-induced acute ulcer model (single dose, whey protein hydrolysate inhibited the gastric lesion index by 65.5% and the double dose resulted in a 77.4% inhibition. CONCLUSION: For the anti-inflammatory model, the cytoprotective effect of low molecular weight peptides was stronger than that of total hydrolysate (53.1 and 71.6%, ulcerative lesion index for single and double dose, respectively. No mucosa cytoprotective activity was found for whey protein concentrate, whey protein hydrolysate or WPHP in the immobilization and cold stress model.

    Full Text Available As larvas da Dermatobia hominis provocam lesões ulcerativas, danificando o tecido subcutâneo e conseqüentemente a pele do hospedeiro. O couro é o subproduto que sofre maior depreciação, o que, muitas vezes, impossibilita seu aproveitamento na industrialização. Atualmente o controle químico é utilizado como forma de combate à dermatobiose, entretanto, leva ao acúmulo de substâncias tóxicas nos animais e no ambiente. No presente trabalho, foram avaliadas as respostas imune-humoral e celular de bovinos imunizados com extrato antigênico preparado com larvas de D. hominis. Três grupos de oito bezerras da raça Nelore com 10 meses de idade foram usados, tendo o primeiro grupo (A recebido aplicação de extrato imunogênico de larvas de D. hominis, com intervalos de quinze dias; o grupo (B, utilizado como controle, não recebendo nenhum tipo de tratamento; e o grupo (C recebendo o tratamento ectoparasiticida à base de Dichlorvos associado a Cypermetrina. Neste mesmo período, foram avaliados o leucograma e os níveis de IgG contra D. hominis pela técnica de enzimoimunoensaio-ELISA. Quanto à avaliação da imunidade humoral, verificou-se que os animais do grupo A apresentaram maior produção de IgG contra D. hominis, com níveis máximos de anticorpos circulantes aos 45 dias após a primeira imunização. Estes animais também apresentaram maior produção de neutrófilos, eosinófilos e monócitos que os dos grupos B e C. O número de nódulos de larvas encontrado nos animais do grupo C foi 148,3% maior que nos animais dos grupos A e B.     Full Text Available OBJETIVO: Estudar as características clínicas, tratamento e evolução de pacientes com acometimento da córnea e esclera associados a doenças do tecido conectivo. MÉTODO: Descrição das alterações de segmento anterior em nove pacientes com doenças do tecido conectivo, previamente diagnosticada (5 casos ou no qual o acometimento ocular foi sua primeira manifestação (4 casos. Todos os pacientes foram atendidos no setor de Doenças Externas Oculares e Córnea da Universidade Federal de São Paulo - Escola Paulista de Medicina (UNIFESP-EPM, acompanhados no ambulatório no período de julho/1999-dezembro/2000 e submetidos a exame oftalmológico completo, avaliação sistêmica e investigação laboratorial. Tratamento clínico e/ou cirúrgico foi proposto de acordo com a gravidade e evolução das manifestações oculares. RESULTADOS: Os diagnósticos sistêmicos observados nos portadores de doença inflamatória do segmento anterior foram de artrite reumatóide em sete pacientes (77,8%, esclerose sistêmica e granulomatose de Wegener em um paciente cada (22,2%. As manifestações oculares mais freqüentes foram esclerite (66,6%, ceratite ulcerativa periférica (55,5% e ceratoconjuntivite seca (44,4%. Oitenta e nove por cento dos pacientes necessitaram de terapia imunossupressora sistêmica para o controle da inflamação ocular. A remissão da inflamação observou-se no período de três meses após o início do imunos-supressor (metotrexato, ciclofosfamida e/ou ciclosporina A. Em 55,5% dos pacientes uma abordagem cirúrgica (ressecção da conjuntiva do limbo, adesivo tecidual com lente de contato terapêutica, enxerto de esclera, transplante de córnea se fez necessária. CONCLUSÃO: O envolvimento da córnea e da esclera associado à doença do tecido conectivo é sinal de atividade da doença e geralmente necessita de terapia imunossupressora para o seu controle. However, in 10 to 25% of patients, the immune system restoration will provoke severe reaction against co-existent infections, causing diseases, by opportunistic agents, with atypical features, and intense tissue inflammation. All clinical and laboratorial changes resulting from this increased inflammatory response are called immune reconstitution inflammatory syndrome (IRIS. Paradoxical clinic worsening of a known disease or appearance of a new one, after beginning of HAART, characterize this syndrome. Potential mechanisms include partial immune reconstitution or host increased immune response to the antigenic stimulus. There are two main forms: an earlier that arises up to the third month after HAART, due to immune reaction against opportunistic agents that remained in a sub-clinic disease; and other later that emerges after months or years as an immune reaction evolution against opportunistic agents whose manifestations were unexpected. This syndrome occurs mainly in those with less than 50/mm³ T CD4 and very high HIV viral load before HAART, as so non-detected antigens from micro-organisms whose clinical manifestations were unexpected. Most diseases are dermatological, mainly, genital herpes and warts.     Full Text Available FUNDAMENTOS: A síndrome de Sweet corresponde a um conjunto de alterações cutâneas, sistêmicas e histopatológicas como resposta a diversos estímulos, semelhantes ao eritema nodoso, ao eritema multiforme e à vasculite leucocitoclásica. São descritas condições associadas como infecção, gravidez, uso de drogas e malignidades. OBJETIVOS: Avaliar as alterações clínicas e histopatológicas da síndrome, relacionando-a a outras condições. MÉTODO: Estudo retrospectivo de 73 casos com avaliação microscópica de lesões cutâneas, dados clínicos e laboratoriais. RESULTADOS: Houve predomínio de mulheres (83,0%, brancas (49,2%, entre a quarta e a sexta décadas de vida (73,8%. Placas eritematosas (76,9%, pápulas (43,0%, pseudovesiculação (PV (38,4% e lesões em alvo (18,5% foram as principais alterações. Entre as condições associadas, infecções de vias aéreas (15,4% e uso de drogas (10,8% foram as mais frequentes. Outras associações, representadas por um caso cada, foram: linfoma de Hodgkin, gravidez, colite ulcerativa, policitemia vera e lúpus eritematoso em paciente com Aids. Neutrófilos com leucocitoclasia (98,6%, degeneração do colágeno (87,7%, edema (74,0% e PV (38,4% foram os principais achados microscópicos. Eosinófilos estiveram presentes (41,1% de raros a abundantes e, em geral, não relacionados ao uso de drogas.     Full Text Available Este trabalho investigou a margem de segurança do inibidor COX-2, meloxicam, em cães, enfocando seus efeitos deletérios nas células sangüíneas e no trato gastrintestinal. Para tanto, após uma avaliação clínico-laboratorial, os cães foram distribuídos nos seguintes grupos: I (placebo; n= 3, II (piroxicam; 1,0mg kg-1; n= 5, III (meloxicam; 0,2mg kg-1; n= 5, IV (meloxicam;1,0mg kg-1; n= 5 e V (meloxicam; 2,0mg kg-1; n= 5. Os fármacos foram usados, por via oral, por 16 dias. No 17º dia, repetiu-se o hemograma completo e, então, procedeu-se a eutanásia seguida pela necrópsia. No grupo I, não houve alterações dignas de nota. No grupo II, todos os cães apresentaram episódios moderados de vômito e diarréia. O perfil celular sangüíneo não foi significativamente modificado. Em dois cães, houve redução no hematócrito e na hemoglobina. Na necropsia, observaram-se focos hemorrágicos e lesões gastriduodenais moderadas. A análise microscópica revelou a presença de gastrite e enterite ulcerativa. No grupo III, quatro cães (80% apresentaram vômito e diarréia, sem alteração no perfil celular sangüíneo.     Full Text Available OBJETIVO: Caracterizar a população de profissionais do sexo em atividade em Ribeirão Preto, Brasil, segundo variáveis demográficas e socioeconômicas, e estudar os fatores de risco para doenças sexualmente transmissíveis nessa população. MÉTODOS: Foram visitados todos os locais de prática de prostituição identificados na cidade. Os participantes responderam um questionário que levantava informações sócio-demográficas e fatores de risco para doenças sexualmente transmissíveis. Foi responsável pelas entrevistas uma assistente social que há mais de 5 anos desenvolvia atividades educativas junto aos profissionais do sexo de Ribeirão Preto. RESULTADOS: As 449 prostitutas, 53 travestis e 13 michês estudados representam uma população jovem, de baixa escolaridade, reduzido nível socioeconômico e grande mobilidade espacial. Em relação às prostitutas, os travestis apresentaram risco significativamente mais elevado para doenças sexualmente transmissíveis, o qual se traduz por diferenças em termos do tempo de trabalho, número médio de parceiros por dia, antecedente de doenças sexualmente transmissíveis ulcerativas, prática de sexo anal, uso de drogas ilícitas não-injetáveis, especialmente o crack, e antecedente de prisão. A exposição a bebidas alcoólicas foi o único fator de risco mais freqüente entre as prostitutas. O uso de preservativo com parceiros fixos foi menos comum do que nas relações sexuais comerciais, tanto em prostitutas como em travestis. CONCLUSÕES: Os profissionais do sexo de Ribeirão Preto, especialmente os travestis, representam uma população socialmente marginalizada e de alto risco para doenças sexualmente transmissíveis. The project involves Colombia, Costa Rica, Cuba, Guatemala, Haiti, Honduras, Jamaica, Mexico, Nicaragua, Panama, Dominican Republic and Venezuela, with the support of the Centre for Energy, Environment and Technology (CIEMAT, Spain), the Institute Research for Development (IRD, France) and in collaboration with the Regional Coordinating Unit of the Caribbean of the United Nations Program for the Environment (UNEP-CAR/RCU). It is expected that the project results will improve the commitment of Member States and regional organizations in activities to protect the environment now and in the future. Through the regional laboratory training and a better understanding of coastal ecosystems from baseline studies and historical reconstruction of pollution levels, a strong network of laboratories with improved capabilities will be able to continue to contribute to the sustainable management of Caribbean Sea. Methodologies are being developed for monitoring and management. The results will be disseminated to the scientific community, policy makers and society, in addition to international organizations such as UNEP and the Association of Caribbean States. The objective of the results is to support integrated management of the coastal zone. One of the main methodologies of this project is the use of coastal sediments as records of environmental change, including pollution.     caso das primeiras. 3 Depois do 5º ano de molestia, a resposta á superinoculação traduziu um estado de maior sensibilidade do organismo infectado. Observou-se no ponto inoculado uma reação precoce papulo-eritemato-ulcerosa, francamente necrotica e destrutiva, ao mesmo tempo que se verificou exacerbação das lesões dos pacientes, com grande infartamento gangliomar satélite. Até 18 meses depois, em um caso, a lesão obtida permaneceu localizada sem manifestações generalizadas. a O tratamento não modificou tal estado. Doentes tratados ( e parcial ou totalmente curados, reagiram da mesma maneira à reinoculação dentro desse período da molestia. Apenas em um caso de mais de 5 anos (nº 40 não se obteve a lesão ulcero-necrotica. Era o único dos 13 experimentados que não tinha nem tivera lesões ulcero-gomoides destrutivas. por outro lado, 2 pacientes apresentando lesões gomo-ulcerativas, mas tendo menos de 3 anos de molestia, não deram a lesão ulcerativo-necrotica em resposta à superinoculação. (Experiências ns. 38 e 39. Interessante é que esta lesão ulcerativo-necrotica contém treponemas embora raros, e em evolução pode tomar o carater das lesões destrutivas gomo-ulcerativas peculiares ao chamado "período terciario" da doença, com as quais também se assemelha histopatológicamente. Sob o ponto de vista imunológico, esta lesão representa um estado de maior sensibilidade do organismo para o agente infeccioso. 4 Como o tratamento precoce perturba o desenvolvimento da imunidade, sob o ponto de vista epidemiológico, seria aconselhável aguardar o período terminal do chamado secundarismo, isto é da fase de generalização boubatica, para tratar os pacientes em Postos, hospitais ou ambulatórios pois, tais doentes poderiam se reinfestar uma vez retornados ao fóco.
