    Orthophotographs have long been recognized for their value as supplements or alternatives to standard maps. Recent trends towards digital cartography have resulted in efforts by the US Geological Survey to develop a digital orthophotoquad production system. Digital image files were created by scanning color infrared photographs on a microdensitometer. Rectification techniques were applied to remove tile and relief displacement, thereby creating digital orthophotos. Image mosaicking software was then used to join the rectified images, producing digital orthophotos in quadrangle format.

    Full Text Available Irish educational policy acknowledges the importance of integrating technology meaningfully into teaching and learning at the primary level. Much remains to be done, however, in terms of developing, practically implementing and evaluating the use of technology in the Irish primary classroom. This paper describes a study on the use of a structured approach to digital storytelling as a tool for meaningful technology integration with children at primary level. In this study, the researcher developed a structured approach for implementing digital storytelling with primary school children, and collaborated with a class teacher in an Irish primary school to prepare to implement the approach. Multiple qualitative methods were used to evaluate the implementation, including interviews with the teacher, classroom observation, and focus group interviews with participating children. The results suggested that this structured approach to digital storytelling has the potential to be used successfully as a meaningful technology integration approach with this cohort. The findings also explored implications for the implementation of digital storytelling at primary level, and identified aspects on which further research should be undertaken to enhance and extend the approach that was used.

    Full Text Available This exploratory research explores the concept of a digital legacy as a general concept and as a collection of digital possessions with unique characteristics. The results reported in this article are part of a larger study. In Cushing (2013, the author identified the characteristics of a digital possession. In this study, these characteristics of a digital possession were utilized to explore how the characteristics of several digital possessions could form a collection, or a digital legacy. In addition to being explored as a collection of digital possessions, data was collected about the general concept of a digital legacy. In part I of the study, 23 participants from three age groups were interviewed about their general concept of a digital legacy. Five general characteristics describing a digital legacy were identified. In part II of the study, interview data from Cushing (2013 was used to create statements describing digital possessions. The statements were classified utilizing the archival concept of primary and secondary values, as well as the consumer behavior concepts of self extension to possessions and possession attachment. Primary value refers to the purpose for which the item was created, while secondary value refers to an additional value that the participants can perceive the item to hold, such as a perception that an item can represent one's identity. Using standard Q method procedure, 48 participants were directed to rank their agreement with 60 statements (written on cards, along a distribution of -5 to +5, according to the characteristics of the digital possession they would most like to maintain for a digital legacy. The ranked statements were analyzed using Q factor analysis, in order to perceive the most common statements associated with maintaining digital possessions for a digital legacy.   7. Digital pathology for the primary diagnosis of breast histopathological specimens: an innovative validation and concordance study on digital pathology validation and training.

    To train and individually validate a group of breast pathologists in specialty-specific digital primary diagnosis by using a novel protocol endorsed by the Royal College of Pathologists' new guideline for digital pathology. The protocol allows early exposure to live digital reporting, in a risk-mitigated environment, and focuses on patient safety and professional development. Three specialty breast pathologists completed training in the use of a digital microscopy system, and were exposed to a training set of 20 challenging cases, designed to help them identify personal digital diagnostic pitfalls. Following this, the three pathologists viewed a total of 694 live, entire breast cases. All primary diagnoses were made on digital slides, with immediate glass slide review and reconciliation before final case sign-out. There was complete clinical concordance between the glass and digital impression of the case in 98.8% of cases. Only 1.2% of cases had a clinically significant difference in diagnosis/prognosis on glass and digital slide reads. All pathologists elected to continue using the digital microscope as the standard for breast histopathology specimens, with deferral to glass for a limited number of clinical/histological scenarios as a safety net. Individual training and validation for digital primary diagnosis allows pathologists to develop competence and confidence in their digital diagnostic skills, and aids safe and responsible transition from the light microscope to the digital microscope. © 2017 John Wiley & Sons Ltd.

    This paper seeks to theorise primary teachers' degree of integration of digital technology in the mathematics classroom. In an age where digital technology use is ubiquitous, the issues surrounding teachers' choice, and ultimately their uptake of digital technologies in the classroom, is an area that need to be further unpacked. Cross-case analysis of the two teachers' uptake of digital technologies in their classroom, their pedagogical approaches and the reason for their choices provide insight into teachers' technological, pedagogical and content knowledge (TPACK). Differences in the way the teachers use digital technology in their classroom seem to be connected to their TPACK developmental stage.

    The intent of this publication is twofold: (1) to serve as a user guide for soil scientists and others interested in learning about the value and use of digital elevation model (DEM) data in making soil surveys and (2) to provide documentation of the Soil Landscape Analysis Project (SLAP). This publication provides a step-by-step guide on how digital slope-class maps are adjusted to topographic maps and orthophotoquads to obtain accurate slope-class maps, and how these derivative maps can be used as a base for soil survey premaps. In addition, guidance is given on the use of aspect-class maps and other resource data in making pre-maps. The value and use of tabular summaries are discussed. Examples of the use of DEM products by the authors and by selected field soil scientists are also given. Additional information on SLAP procedures may be obtained from USDA, Soil Conservation Service, Soil Survey Division, P.O.     Primary intestinal lymphangiectasia (PIL), also known as Waldmann's disease, is a rare disorder characterized by dilated intestinal lacteals resulting in lymph leakage into the small bowel lumen and responsible for protein-losing enteropathy leading to lymphopenia, hypoalbuminemia and hypogammaglobulinemia. The symptoms usually start in early infancy. We report a case of secondary hyperparathyroidism, osteopenia, monoclonal gammopathy and digital clubbing in a 57-year-old patient with a 12-year history of discontinuous diarrhea. Malabsorption with inability to gain weight, and finally weight loss and formation of leg edema were associated with protein-losing enteropathy. A low-fat diet associated with medium-chain triglyceride supplementation was clinically effective as medical management in reducing diarrhea and leg edema, and promoting weight gain. Double-balloon enteroscopy and small bowel biopsy histopathology confirmed dilated intestinal lacteals. Digital clubbing associated with primary intestinal lymphangiectasia which may causally be related to chronic platelet excess has not been reported before.

  12. Why primary care practices should become digital health information hubs for their patients.

    Two interesting health care trends are currently occurring: 1) patient-facing technologies, such as personal health records, patient portals, and mobile health apps, are being adopted at rapid rates, and 2) primary care, which includes family practice, is being promoted as essential to reducing health care costs and improving health care outcomes. While these trends are notable and commendable, both remain subject to significant fragmentation and incentive misalignments, which has resulted in significant data coordination and value generation challenges. In particular, patient-facing technologies designed to increase care coordination, often fall prey to the very digital fragmentation issues they are supposed to overcome. Additionally, primary care providers are treating patients that may have considerable health information histories, but generating a single view of such multi-source data is nearly impossible. We contribute to this debate by proposing that primary care practices become digital health information hubs for their patients. Such hubs would offer health data coordination in a medically professional setting with the benefits of expert, trustworthy advice coupled with active patient engagement. We acknowledge challenges including: costs, information quality and provenance, willingness-to-share information and records, willingness-to-use (by both providers and patients), primary care scope creep, and determinations of technical and process effectiveness. Even with such potential challenges, we strongly believe that more debate is needed on this topic prior to full implementation of various health information technology incentives and reform programs currently being designed and enacted throughout the world. Ultimately, if we do not provide a meaningful way for the full spectrum of health information to be used by both providers and patients, especially early in the health care continuum, effectively improving health outcomes may remain elusive.     Full Text Available Digital textbooks were introduced in the Romanian education system in the 2014- 2015 school year. In order to find out how well prepared were the primary grades teachers and pupils to use the six digital textbooks of Mathematics and Environmental Exploration, for the 2nd grade, we administered a questionnaire. To the questionnaire, with 14 items, 36 primary grades teachers answered on a voluntary basis. We analysed teachers’ information and opinions about the following: the context in which they achieved knowledge about the digital textbooks, how adequate the respective technology was, the appropriateness of infrastructure and human resource for using the respective textbooks, the electronic support and the electronic apparatuses used, the context of using them and the forms of organising the activities for their use, the frequency of their use during classes, and the importance of working with those digital textbooks, as well as some of their disadvantages.

  15. Digital Radiography for Determination of Primary Tooth Length: In Vivo and Ex Vivo Studies

    Full Text Available Background. Methods for determining the root canal length of the primary tooth should yield accurate and reproducible results. In vitro studies show some limitations, which do not allow their findings to be directly transferred to a clinical situation. Aim. To compare the accuracy of radiographic tooth length obtained from in vivo digital radiograph with that obtained from ex vivo digital radiograph. Method. Direct digital radiographs of 20 upper primary incisors were performed in teeth (2/3 radicular resorption that were radiographed by an intraoral sensor, according to the long-cone technique. Teeth were extracted, measured, and mounted in a resin block, and then radiographic template was used to standardise the sensor-target distance (30 cm. The apparent tooth length (APTL was obtained from the computer screen by means of an electronic ruler accompanying the digital radiography software (CDR 2.0, whereas the actual tooth length (ACTL was obtained by means of a digital calliper following extraction. Data were compared to the ACTL by variance analysis and Pearson’s correlation test. Results. The values for APTL obtained from in vivo radiography were slightly underestimated, whereas those values obtained from ex vivo were slightly overestimated. No significance was observed (P≤0.48 between APTL and ACTL. Conclusion. The length of primary teeth estimated by in vivo and ex vivo comparisons using digital radiography was found to be similar to the actual tooth length.

  16. Primary breast osteosarcoma mimicking calcified fibroadenoma on screening digital breast tomosynthesis mammogram

    Full Text Available Primary breast osteosarcoma is a rare malignancy, with mostly case reports in the literature. The appearance of breast osteosarcoma on digital breast tomosynthesis imaging has not yet been described. A 69-year-old woman presents for routine screening mammography and is found to have a calcified mass in her right breast. Pattern of calcification appeared “sunburst” on digital breast tomosynthesis images. This mass was larger than on the previous year's mammogram, at which time it had been interpreted as a benign calcified fibroadenoma. The subsequent workup demonstrated the mass to reflect primary breast osteosarcoma. The patient's workup and treatment are detailed in this case. Primary breast osteosarcoma, although rare, should be included as a diagnostic consideration for breast masses with a sunburst pattern of calcifications, particularly when the mammographic appearance has changed.

    Full Text Available This article contributes to academic discussions on how digital storytelling in an educational setting may have potential to build and develop learning identities, agency and digital competences. With a socio-cultural framework on learning and identity as a point of departure, the article sets out to study these issues approached as boundary crossing between the intersecting contexts of leisure time and school. The analysis draws on three examples of digital storytelling among 5th - 7th graders in three Norwegian primary school classes. My findings suggest that digital storytelling might represent a boundary crossing enabling pupils to adopt new roles as producers of creative content, as mentors or guides, to explore new technology and software in a context different from that of outside school and to learn and develop competences related to production processes and multimodal resources. I argue that digital storytelling has a potential to contribute to learning, learning identity and agency, provided it is based on a more fully developed pedagogical strategy of carefully linking school and leisure time.

    Full Text Available This paper presents the findings of a study carried out in the academic year 2014-2015 at the faculty of Education of the University of Murcia with first year degree students in Primary Education studying Research and ICT. The study started with the application of the DIGCOM questionnaire to analyze the digital competences of 134 students. The questionnaire served as an initial task to help students reflect on their digital competences. The subject was developed around tasks which adopted a transversal approach and used the nature of the contents itself to direct and improve students’ digital competencies. Finally, the initial questionnaire was reformulated and run in order to ascertain the students’ self-perception of their improvement in these competencies through the tasks they had performed. Below we present the tasks carried out, the organization of each subject and the most relevant data regarding the self-perception of digital competencies of the future primary school teachers enrolled at the University of Murcia. The data reveal, on the one hand, that the students participating consider themselves to be competent in the most basic aspects of digital competencies and, on the other, their perception that the work done in the subject has helped them quite a lot in improving their competencies.

    Full Text Available Aim: The purpose of this study was to compare the Working length in primary teeth endodontics using intra oral digital radiovisiography and apex locator with conventional method for accuracy. Materials and Methods: This in vivo study was conducted on 30 primary teeth which were indicated for pulpectomy in the patients of the age group of 5-11 years All experimental teeth had adequate remaining tooth structure for rubber dam isolation and radiographicaly visible canals. Endodontic treatment was required due to irreversible pulpitis or pulp necrosis. A standardized intraoral periapical radiograph of the tooth was taken using conventional method by paralleling technique. The distance between the source and the tooth, tooth and the films were standardized using X-ray positioning device. During the pulpectomy procedure, the working length was determined by digital radiograph and apex locator. The measurements were then compared with the conventional method of root canal measurement technique for accuracy Result: From the results obtained we can conclude that Working length determined in primary molars using digital radiography and Apex locator did not show any significant difference in the mean working length measurements when compared with the conventional radiographic method. Conclusions: Apex locator is comparable to conventional radiograph in determining the working length without radiation in the primary teeth. Intraoral digital radiography is the safest method in determining the working length with significant reduction in radiation exposure.Hence, both the techniques can be safely used as alternatives to conventional radiographic methods in determining working length in primary teeth.

    Nurses have expressed doubts about the ongoing digitalisation of Swedish primary health care. Given the potential role of eHealth in primary health care, including supporting interactive self-management for people with chronic conditions, it is important to highlight nurses' experiences. This study is part of a larger project aimed at implementing person-centred interactive self-management support (iSMS) in primary health care. The aim of this study was to describe Swedish primary healthcare nurses' perceptions of using digital eHealth systems and services to support patient self-management. Focus group interviews were conducted with primary healthcare nurses (n = 20). The interview transcriptions were analysed using qualitative content analysis. Three themes emerged from the content analysis: caregiving in the midst of digital chaos; a lack of overview and control in daily work; and mixed feelings towards digitalisation. Each theme was subdivided into three subthemes. The results of this study provide insight into a number of concerns that stand in the way of success when it comes to the implementation and use of digital technology. If nurses are to adapt to the new policies and practices that accompany the current digitalised development in Swedish primary health care, the concept of a nurse's traditional work role needs to be amended in terms of the scope of work tasks and established views of traditional nursing. The study also highlights the need for more research to enable eHealth systems/services to be designed to fulfil multiple requirements. The digitised systems should be a tool for achieving good quality self-management support as well as giving the primary healthcare nurses adequate resources to support patients' self-management while still maintaining the values associated with person-centred care. © 2017 Nordic College of Caring Science.

    In order to monitor the region which disaster flood happened frequently in China, satisfy the great need of province governments for high accuracy monitoring and evaluated data for disaster and improve the efficiency for repelling disaster, under the Ninth Five-year National Key Technologies Programme, the method was researched for flood forecasting and evaluation using satellite and aerial remoted sensed image and land monitor data. The effective and practicable flood forecasting and evaluation system was established and DongTing Lake was selected as the test site. Modern Digital photogrammetry, remote sensing and GIS technology was used in this system, the disastrous flood could be forecasted and loss can be evaluated base on '4D' (DEM -- Digital Elevation Model, DOQ -- Digital OrthophotoQuads, DRG -- Digital Raster Graph, DTI -- Digital Thematic Information) disaster background database. The technology of gathering and establishing method for '4D' disaster environment background database, application technology for flood forecasting and evaluation based on '4D' background data and experimental results for DongTing Lake test site were introduced in detail in this paper.

  10. Digit ratio (2D:4D) in primary brain tumor patients: A case-control study.

    The second-to-fourth digit ratio (2D:4D) reflects prenatal estrogen and testosterone exposure, and is established in utero. Sex steroids are implicated in development and progression of primary brain tumors. To investigate whether there is a link between 2D:4D ratio and primary brain tumors, and age at presentation. Digital images of the right and left palms of 85 primary brain tumor patients (age 56.96±13.68years; 71% women) and 106 (age 54.31±13.68years; 68% women) gender and age matched controls were obtained. The most common brain tumor diagnoses were meningioma (41%), glioblastoma (20%) and pituitary adenoma (16%). Right and left 2D:4D ratios, and right minus left 2D:4D (D r-l ) were compared between patients and controls, and were correlated with age. Right and left 2D:4D ratios were significantly lower in primary brain tumor patients relative to controls (t=-4.28, pbrain tumor patients and controls (p=0.27). In meningioma and glioma patients, age at presentation correlated negatively with left 2D:4D ratio (rho=-0.42, p=0.01 and rho=-0.36, p=0.02, respectively) and positively with D r-l (rho=0.45, p=0.009 and rho=0.65, p=0.04, respectively). Right and left hand 2D:4D ratios are lower in primary brain tumor patients relative to healthy individuals suggesting greater prenatal testosterone and lower prenatal estrogen exposure in brain tumor patients. Greater age at presentation is associated with greater D r-l and with lower left 2D:4D ratio of meningioma and glioma patients.   11. Track: A randomized controlled trial of a digital health obesity treatment intervention for medically vulnerable primary care patients.

    Obesity continues to disproportionately affect medically vulnerable populations. Digital health interventions may be effective for delivering obesity treatment in low-resource primary care settings. Track is a 12-month randomized controlled trial of a digital health weight loss intervention in a community health center system. Participants are 351 obese men and women aged 21 to 65years with an obesity-related comorbidity. Track participants are randomized to usual primary care or to a 12-month intervention consisting of algorithm-generated tailored behavior change goals, self-monitoring via mobile technologies, daily self-weighing using a network-connected scale, skills training materials, 18 counseling phone calls with a Track coach, and primary care provider counseling. Participants are followed over 12months, with study visits at baseline, 6, and 12months. Anthropometric data, blood pressure, fasting lipids, glucose and HbA1C and self-administered surveys are collected. Follow-up data will be collected from the medical record at 24months. Participants are 68% female and on average 50.7years old with a mean BMI of 35.9kg/m(2). Participants are mainly black (54%) or white (33%); 12.5% are Hispanic. Participants are mostly employed and low-income. Over 20% of the sample has hypertension, diabetes and hyperlipidemia. Almost 27% of participants currently smoke and almost 20% score above the clinical threshold for depression. Track utilizes an innovative, digital health approach to reduce obesity and chronic disease risk among medically vulnerable adults in the primary care setting.     The Statement by the Spanish Society of Family and Community Medicine (SemFYC) on access to scientific information, highlights the need for providing digital libraries with certain resources in Autonomous Regions. The primary goal is to study the evidence-based medicine (EBM) coverage that SemFYC recommends regional virtual libraries. The regional health virtual libraries were identified and the access provided to health professionals, Internet presence, remote access and resources were studied. The results suggest there is ample coverage in 8 Autonomous Regions. At the top of the list was, Health Sciences Virtual Library of Navarre, the Balearic Islands Health Sciences Virtual Library, and Virtual Library of the Andalusian Public Health System.     The SPECTRa (Submission, Preservation and Exposure of Chemistry Teaching and Research Data) project has investigated the practices of chemists in archiving and disseminating primary chemical data from academic research laboratories. To redress the loss of the large amount of data never archived or disseminated, we have developed software for data publication into departmental and institutional Open Access digital repositories (DSpace). Data adhering to standard formats in selected disciplines (crystallography, NMR, computational chemistry) is transformed to XML (CML, Chemical Markup Language) which provides added validation. Context-specific chemical metadata and persistent Handle identifiers are added to enable long-term data reuse. It was found essential to provide an embargo mechanism, and policies for operating this and other processes are presented.

    Full Text Available Digital services are often regarded as a solution to the growing demands on primary care services. Provision of a tool offering advice to support self-management as well as the ability to digitally consult with a General Practitioner (GP has the potential to alleviate some of the pressure on primary care. This paper reports on a Phase II, 6-month evaluation of eConsult, a web-based triage and consultation system that was piloted across 11 GP practices across Scotland. Through a multi-method approach the evaluation explored eConsult use across practices, exposing both barriers and facilitators to its adoption. Findings suggest that expectations that eConsult would offer an additional and alternative method of accessing GP services were largely met. However, there is less certainty that it has fulfilled expectations of promoting self-help. In addition, low uptake meant that evaluation of current effectiveness was difficult for practices to quantify. The presence of an eConsult champion(s within the practice was seen to be a significant factor in ensuring successful integration of the tool. A lack of patient and staff engagement, insufficient support and lack of protocols around processes were seen as barriers to its success.

  2. Scatter correction method with primary modulator for dual energy digital radiography: a preliminary study

    In conventional digital radiography (DR) using a dual energy subtraction technique, a significant fraction of the detected photons are scattered within the body, resulting in the scatter component. Scattered radiation can significantly deteriorate image quality in diagnostic X-ray imaging systems. Various methods of scatter correction, including both measurement and non-measurement-based methods have been proposed in the past. Both methods can reduce scatter artifacts in images. However, non-measurement-based methods require a homogeneous object and have insufficient scatter component correction. Therefore, we employed a measurement-based method to correct for the scatter component of inhomogeneous objects from dual energy DR (DEDR) images. We performed a simulation study using a Monte Carlo simulation with a primary modulator, which is a measurement-based method for the DEDR system. The primary modulator, which has a checkerboard pattern, was used to modulate primary radiation. Cylindrical phantoms of variable size were used to quantify imaging performance. For scatter estimation, we used Discrete Fourier Transform filtering. The primary modulation method was evaluated using a cylindrical phantom in the DEDR system. The scatter components were accurately removed using a primary modulator. When the results acquired with scatter correction and without correction were compared, the average contrast-to-noise ratio (CNR) with the correction was 1.35 times higher than that obtained without correction, and the average root mean square error (RMSE) with the correction was 38.00% better than that without correction. In the subtraction study, the average CNR with correction was 2.04 (aluminum subtraction) and 1.38 (polymethyl methacrylate (PMMA) subtraction) times higher than that obtained without the correction.     PhotoExam is a mobile app that incorporates digital photographs into the electronic health record (EHR) using iPhone operating system (iOS, Apple Inc)-based mobile devices. The aim of this study was to describe usage patterns of PhotoExam in primary care and to assess clinician-level factors that influence the use of the PhotoExam app for teledermatology (TD) purposes. Retrospective record review of primary care patients who had one or more photos taken with the PhotoExam app between February 16, 2015 to February 29, 2016 were reviewed for 30-day outcomes for rates of dermatology consult request, mode of dermatology consultation (curbside phone consult, eConsult, and in-person consult), specialty and training level of clinician using the app, performance of skin biopsy, and final pathological diagnosis (benign vs malignant). During the study period, there were 1139 photo sessions on 1059 unique patients. Of the 1139 sessions, 395 (34.68%) sessions documented dermatologist input in the EHR via dermatology curbside consultation, eConsult, and in-person dermatology consult. Clinicians utilized curbside phone consults preferentially over eConsults for TD. By clinician type, nurse practitioners (NPs) and physician assistants (PAs) were more likely to utilize the PhotoExam for TD as compared with physicians. By specialty type, pediatric clinicians were more likely to utilize the PhotoExam for TD as compared with family medicine and internal medicine clinicians. A total of 108 (9.5%) photo sessions had a biopsy performed of the photographed site. Of these, 46 biopsies (42.6%) were performed by a primary care clinician, and 27 (25.0%) biopsies were interpreted as a malignancy. Of the 27 biopsies that revealed malignant findings, 6 (22%) had a TD consultation before biopsy, and 10 (37%) of these biopsies were obtained by primary care clinicians.   4. Introducing a telemonitoring platform for diabetic patients in primary care: will it increase the socio-digital divide?

    This study investigates whether diabetes patients visiting a primary care setting are interested in using a telemonitoring platform and if so, whether characteristics of interested users could be distinguished. Three questionnaires were administered by 92 diabetes persons recruited between May and September 2011. Descriptive statistics and logistic regression analysis were performed. Special attention was drawn to include patients with low educational levels. Patients with middle or high educational levels show quite some interest in the use of a telemonitoring platform, especially for the transmission of glycaemic data or for asking questions. Patients with low educational levels only show a minor interest in using such a platform. It is possibly worthwhile to implement a telemonitoring platform in a primary care setting; however this study did not show immediate profit for implementation in a CHC that organises diabetes clinics on regular basis. In primary care settings where it will be implemented, even if there is a social-digital divide today, the use of a telemonitoring platform could possibly reduce inequity in health care as time could become available for those most in need for face-to-face contact with their physician. Con este dispositivo el proyecto pretende mejorar las competencias digitales de los niños a partir de 7-8 años de edad, que son, ya, usuarios de Internet.

    The radioactivity measurement techniques developed within the EURAMET EMRP 'MetroFission' Joint Research Project, were aimed at performing on-site activity measurements at the primary standard level (4πβ-γ coincidence counting) for a wide range of radionuclides utilizing recent advances in high-speed digital sampling and digital signal processing. The state-of-the-art technology employed within this project provides up to 14-bit digitizer systems operating with sampling rates in the order of 10 8 to 10 9 samples-per-second, incorporating on-board FPGA devices, which greatly enhances the application of digital signal processing for the implementation of digital coincidence counting. These devices when coupled to suitable analysis software, demonstrate a significant improvement in the provision of primary standards of radioactivity. This manuscript provides a description of the systems employed, along with recommendations regarding optimization of the digital sampling of signals from photo-multiplier tubes and pre-amplifiers and compare the benefits of 'off-line' versus 'on-line' 4πβ-γ digital coincidence counting systems.     AREVA NP has gained significant experience during the past five years in digital upgrades at operating nuclear power stations in the US. Plants are seeking modernization with digital technology to address obsolescence, spare parts availability, vendor support, increasing age-related failures and diminished reliability. New systems offer improved reliability and functionality, and decreased maintenance requirements. Significant lessons learned have been identified relating to the areas of licensing, equipment qualification, software quality assurance and other topics specific to digital controls. Digital control systems have been installed in non safety-related control applications at many utilities within the last 15 years. There have also been a few replacements of small safety-related systems with digital technology. Digital control systems are proving to be reliable, accurate, and easy to maintain.     Letter recognition and digit recognition are critical skills for literate adults, yet few studies have considered the development of these skills in children. We conducted a nine-alternative forced-choice (9AFC) partial report task with strings of letters and digits, with typographical symbols (e.g., $, @) as a control, to investigate the development of identity and position processing in children. This task allows for the delineation of identity processing (as overall accuracy) and position coding (as the proportion of position errors). Our participants were students in Grade 1 to Grade 6, allowing us to track the development of these abilities across the primary school years. Our data suggest that although digit processing and letter processing end up with many similarities in adult readers, the developmental trajectories for identity and position processing for the two character types differ. Symbol processing showed little developmental change in terms of identity or position accuracy. We discuss the implications of our results for theories of identity and position coding: modified receptive field, multiple-route model, and lexical tuning. Despite moderate success for some theories, considerable theoretical work is required to explain the developmental trajectories of letter processing and digit processing, which might not be as closely tied in child readers as they are in adult readers.     Purpose. To assess the suitability of digital stereo images for optic disc evaluations in glaucoma. Methods. Stereo color optic disc images in both digital and 35-mm slide film formats were acquired contemporaneously from 29 subjects with various cup-to-disc ratios (range, 0.26–0.76; median, 0.475). Using a grading scale designed to assess image quality, the ease of visualizing optic disc features important for glaucoma diagnosis, and the comparative diameters of the optic disc cup, experienced observers separately compared the primary digital stereo images to each subject's 35-mm slides, to scanned images of the same 35-mm slides, and to grayscale conversions of the digital images. Statistical analysis accounted for multiple gradings and comparisons and also assessed image formats under monoscopic viewing. Results. Overall, the quality of primary digital color images was judged superior to that of 35-mm slides (P digital color images were mostly equivalent to the scanned digitized images of the same slides. Color seemingly added little to grayscale optic disc images, except that peripapillary atrophy was best seen in color (P digital over film images was maintained under monoscopic viewing conditions. Conclusions. Digital stereo optic disc images are useful for evaluating the optic disc in glaucoma and allow the application of advanced image processing applications. Grayscale images, by providing luminance distinct from color, may be informative for assessing certain features. PMID:20505199

    Digital pathology is becoming technically possible to implement for routine pathology work. At our institution, we have been using digital pathology for second opinion intraoperative consultations for over 10 years. Herein, we describe our experience in converting to a digital pathology platform for primary pathology diagnosis. We implemented an incremental rollout for digital pathology on subspecialty benches, beginning with cases that contained small amounts of tissue (biopsy specimens). We successfully scanned over 40,000 slides through our digital pathology system. Several lessons (both challenges and opportunities) were learned through this implementation. A successful conversion to digital pathology requires pre-imaging adjustments, integrated software and post-imaging evaluations.

    Pulmonary digital subtraction angiography was diagnostic in 98.3% of patients with possible acute pulmonary embolism. The procedure was well tolerated even in severely ill patients. A large image intensifier made simultaneous imaging of both lungs possible reducing the number of contrast injections necessary. Small volumes of low iso-osmolar concentration of modern contrast media were used. There was no need for catherization of the pulmonary artery. Theoretical considerations and our limited experience indicate that this will reduce the number of complications compared with conventional pulmonary angiography. The procedure is rapidly performed and the diagnostic accuracy high. This makes digital subtraction angiography cost effective. Digital pulmonary angiography can be recommended as the primary diagnostic method in most patients with possible pulmonary embolism. (orig.)

  3. Cost Analysis of a Digital Health Care Model in Sweden.

    Digital technologies in health care are expected to increase in scope and to affect ever more parts of the health care system. It is important to enhance the knowledge of whether new digital methods and innovations provide value for money compared with traditional models of care. The objective of the study was to evaluate whether a digital health care model for primary care is a less costly alternative compared with traditional in-office primary care in Sweden. Cost data for the two care models were collected and analyzed to obtain a measure in local currency per care contact. The comparison showed that the total economic cost of a digital consultation is 1960 Swedish krona (SEK) (SEK100 = US$11.29; February 2017) compared with SEK3348 for a traditional consultation at a health care clinic. Cost differences arose on both the provider side and on the user side. The digital health care model may be a less costly alternative to the traditional health care model. Depending on the rate of digital substitution, gross economic cost savings of between SEK1 billion and SEK10 billion per year could be realized if more digital consultations were made. Further studies are needed to validate the findings, assess the types of care most suitable for digital care, and also to obtain various quality-adjusted outcome measures.

    There are only relative indications for distal digital replantation in zones 1 and 2 according to Tamai. In contrast to primary closure for fingertip amputations, replantation is a complex procedure that requires skills in supermicrosurgical techniques, as vessels with diameters between 0.3-0.8 mm are connected. In addition the time spent in hospital and the time off from work are longer. Distal digital replantation is thus only indicated, if the expected functional and aesthetic benefits surmount those of primary closure. We retrospectively analysed all fingertip amputations in zone 1 and 2 according to Tamai between 9/2009 and 7/2014 where we attempted distal digital replantation. The success of replantation, wound healing and functional results were evaluated according to Yamano. We performed 11 distal digital replantations in the study period. There were 6 total amputations, 4 subtotal amputations and 1 avulsion of the digital pulp. Revascularisation with long-term reattachment of the amputated tissues was possible in 8 cases (73%). In 3 cases (27%) secondary amputation closure was necessary. The mean operating time was 3 h 56 min. 6 patients, which had a successful replantation, were available for follow-up examinations after a mean period of 19 months. 5 patients were satisfied with the result and would again prefer replantation over primary amputation closure. 4 patients reported a good function of the replanted digits and did not complain about any limitations in their use. 2 patients complained about restricted function. All patients could return to their previous places of employment and were free of pain. Of the 12 affected digital nerves 11 nerves had a 2-point discrimination (2-PD) of ≤15 mm, 3 of them had a 2-PD between 7 and 10 mm and 4 of them of replanted digits and nail deformities in 2 patients.     The usefulness of digital image processing of chest radiographs was evaluated in a clinical study. In 54 patients, chest radiographs in the posteroanterior projection were obtained by both 14 inch digital image intensifier equipment and the conventional screen-film technique. The digital radiographs (512x512 image format) viewed on a 625 line monitor were processed in 3 different ways: 1.standard display; 2.digital edge enhancement for the standard display; 3.inverse intensity display. The radiographs were interpreted independently by 3 radiologists. Diagnoses were confirmed by CT, follow-up radiographs and clinical records. Chest abnormalities of the films analyzed included 21 primary lung tumors, 44 pulmonary nodules, 16 cases with mediastinal disease, 17 with pneumonia /atelectasis. Interstitial lung disease, pleural plaques, and pulmonary emphysema were found in 30, 18 and 19 cases respectively. Sensitivity of conventional radiography when averaged overall findings was better than that of digital techniques (P<0.001). Differences in diagnostic accuracy measured by sensitivity and specificity between the 3 digital display modes were small. Standard image display showed better sensitivity for pulmonary nodules (0.74 vs 0.66; P<0.05) but poorer specificity for pulmonary emphysema (0.85 vs 0.93; P<0.05) compared with inverse intensity display. It is concluded that when using 512x512 image format, the routine use of digital edge enhancement and tone reversal at digital chest radiographs is not warranted. (author). 12 refs.; 4 figs.; 2 tabs

    This paper outlines the design and testing of a digital imaging system that utilizes an artificial neural network with unsupervised and supervised learning to convert streaming input (real time) image space into parameter space. The primary objective of this work is to investigate the effectiveness of using a neural network to significantly reduce the information density of streaming images so that objects can be readily identified by a limited set of primary parameters and act as an enhanced human machine interface (HMI). Many applications are envisioned including use in biomedical imaging, anomaly detection and as an assistive device for the visually impaired. A digital circuit was designed and tested using a Field Programmable Gate Array (FPGA) and an off the shelf digital camera. Our results indicate that the networks can be readily trained when subject to limited sets of objects such as the alphabet. We can also separate limited object sets with rotational and positional invariance. The results also show that limited visual fields form with only local connectivity.

    This chapter is about enhancing access to primary cultural heritage materials of India housed in academic libraries by integrating them with machine-readable texts, lexical resources, and linguistic software in a digital library. Integrating primary cultural materials with a digital library can enable broad use of Indic collections for research and education. For the purposes of illustrating this procedure, we outline here the development of a prototype using the collections of Sanskrit manuscripts in the libraries at Brown University and the University of Pennsylvania and integrating them with The Sanskrit Library. The result is extendable to collections of Indic materials throughout the world and can serve as a model for digitization projects of cultural materials in other major culture-bearing languages such as Greek, Latin, Arabic, Persian, and Chinese.

    The digital applications in radiology are a controversial advanced which potentially will influence all areas of patient imaging. It is utilized and accepted in angiography, computed tomography, magnetic resonance, nuclear imaging and sonography. More recently Computed Radiography has gained credibility in mobile scenarios as well as specific applications from cervical spine radiography to digital fluoroscopy. Usually this acceptance is related to benefits of lesser radiation exposure or an improved presentation with an incorrect radiographic technique. One advantage of interpreting from digital information is the potential manipulation of the image presentation to the observer through windowing, leveling and edge enhancement pre and/or during image review. Additionally this digital data can be transmitted over distance and represented as hard and/or soft copy for primary or consultative review. The number and quality of the images to be viewed, the environment of the review station as well as the observer experience with conventional radiographic as well as digital image evaluation are important aspects of delivering the radiologist's product i.e. the final interpretation. This paper assesses that product, specifically addressing the question `Is the radiologist's report the same whether derived from the original analog image or from its digitized image.' The object of this study is to determine whether a digital system (3M PACS) designed for consultative viewing in a satellite department can also be used directly for primary diagnosis of conventional chest exams.

  9. Digital stethoscope system: the feasibility of cardiac auscultation

    The application of the digital stethoscope system is a new tendency in methods of cardiac auscultation. Heart sounds, generated by the fluctuations of blood velocity and vibrations of muscle structure, are an important signal in the primary diagnosis of heart diseases. Since the XIXs century for physical examination an analog stethoscope was used, but the development of microelectronics enable the construction of digital stethoscopes which started modern phonocardiography. The typical hardware of the system could be divided into analog and digital parts, respectively. The first one consists of microphone and pre-amplifier. The second one contains a microcontroller with peripherals for data saving and transmission. Usually the specialized software is applied for the signal acquisition and digital signal processing (filtering, spectral analysis and others). This paper presents an overview of methods used in cardiac auscultation and expected developing path in the future. It also contains the description of our digital stethoscope system, which is planned to be used in poliphysiographical studies.

    The growth of digital methods in pathology is accelerating. Digital images can be used for a variety of applications in cytology, including rapid interpretations, primary diagnosis and second opinions, continuing education and proficiency testing. All of these functions can be performed using small static digital images, real-time dynamic digital microscopy, or whole-slide images. This review will discuss the general principles of digital pathology, its methods and applications to cytologic specimens. As cytologic specimens have unique features compared to histopathology specimens, the key differences will be discussed. Technical and administrative issues in digital pathology applications and the outlook for the future of the field will be presented. Copyright © 2011 S. Karger AG, Basel.

  10. A color-communication scheme for digital imagery

    Color pictures generated from digital images are frequently used by geologists, foresters, range managers, and others. These color products are preferred over black and white pictures because the human eye is more sensitive to color differences than to various shades of gray. Color discrimination is a function of perception, and therefore colors in these color composites are generally described subjectively, which can lead to ambiguous color communication. Numerous color-coordinate systems are available that quantitively relate digital triplets representing amounts of red, free, and blue to the parameters of hue, saturation, and intensity perceived by the eye. Most of these systems implement a complex transformation of the primary colors to a color space that is hard to visualize, thus making it difficult to relate digital triplets to perception parameters. This paper presents a color-communcation scheme that relates colors on a color triangle to corresponding values of "hue" (H), "saturation" (S), and chromaticity coordinates (x,y,z). The scheme simplifies the relation between red, green, and blue (RGB) digital triplets and the color generated by these triplets. Some examples of the use of the color-communication scheme in digital image processing are presented.

    Substantial progress could be reached both in system technics and in application of digital acoustic monitoring systems for assessing mechanical integrity of reactor primary systems. For the surveillance of PWRs and BWRs during power operation of the plants, acoustic signals of Loose Parts Monitoring System sensors are continuously monitored for signal bursts associated with metallic impacts. ISTec/GRS experience with its digital systems MEDEA and RAMSES has shown that acoustic signature analysis is very successful for detecting component failures at an early stage. Methods for trending and classification of digital burst signals are shown, experience with their practical use will be presented. (author)

    A program of evaluation of compression algorithms proposed for use in a digital cinema application is described and the results presented in general form. The work was intended to aid in the selection of a compression system to be used for the digital cinema release of Star Wars: Episode II, in May 2002. An additional goal was to provide feedback to the algorithm proponents on what parameters and performance levels the feature film industry is looking for in digital cinema compression. The primary conclusion of the test program is that any of the current digital cinema compression proponents will work for digital cinema distribution to today's theaters.

    Full Text Available The relationships between primary source materials and their digital surrogates warrant consideration about how different materials translate into digitized forms. Physical primary source materials found in library special collections and archives and their digital surrogates challenge the viewer to consider what these objects are communicating through their materiality or lack thereof. For example, how does a clay tablet represent itself digitally, as compared to a parchment manuscript, or a paper accounts book? What qualities, stories or narratives do these resources communicate in their original forms, as digital surrogates, or when engaged with together, and how do these differ? How do both physical and digital resources serve as archival objects with the ability to reflect our social and cultural experiences—and indeed our humanity—back to us? As more and more library and museum resources are digitized and made open to researchers, such questions must be addressed as the use and reuse of digital surrogates becomes increasingly complex as digital scholarship evolves.

  11. Investigating the effect of independent, blinded digital image assessment on the STOP GAP trial.

    Blinding is the process of keeping treatment assignment hidden and is used to minimise the possibility of bias. Trials at high risk of bias have been shown to report larger treatment effects than low-risk studies. In dermatology, one popular method of blinding is to have independent outcome assessors who are unaware of treatment allocation assessing the endpoint using digital photographs. However, this can be complex, expensive and time-consuming. The objective of this study was to compare the effect of blinded and unblinded outcome assessment on the results of the STOP GAP trial. The STOP GAP trial compared prednisolone to ciclosporin in treating pyoderma gangrenosum. Participants' lesions were measured at baseline and at 6 weeks to calculate the primary outcome, speed of healing. Independent blinded assessors obtained measurements from digital photographs using specialist software. In addition, unblinded treating clinicians estimated lesion area by measuring length and width. The primary outcome was determined using blinded measurements where available, otherwise unblinded measurements were used (method referred to as trial measurements). In this study, agreement between the trial and unblinded measurements was determined using the intraclass correlation coefficient (ICC). The STOP GAP trial's primary analysis was repeated using unblinded measurements only. We introduced differential and nondifferential error in unblinded measurements and investigated the effect on the STOP GAP trial's primary analysis. Eighty-six (80%) of the 108 patients were assessed using digital images. Agreement between trial and unblinded measurements was excellent (ICC = 0.92 at baseline; 0.83 at 6 weeks). There was no evidence that the results of the trial primary analysis differed according to how the primary outcome was assessed (p value for homogeneity = 1.00).     Full Text Available The aim of this action research was to investigate the effects of digital storytelling in improving the writing skills of third grade students enrolled in rural primary schools. The writing performances of the students were measured before and after the teaching procedures of digital storytelling. Then, the process of narrative writing with digital storytelling was profoundly and carefully explored through observation and field notes, interviews, audio and video records, student diaries and documents, and student products. The results indicated that digital storytelling enhanced students’ ideas, organization, word choice, sentence fluency, and conventions in terms of writing quality. Similarly, the digital storytelling improved story elements and word counts in stories. In terms of the quality of students’ digital stories, the results demonstrated a steady progress in the elements of digital stories, and the technology literacy and competency of students throughout the process. Besides, the digital storytelling modified the process of narrative writing, and emerged as a beneficial tool to overcome the digital divide by developing students’ new literacy perception, competency, and skills. The digital storytelling also created learning community by improving interactions among students in the classroom, and increased their motivation to write.

    Full Text Available This article presents the implementation of digital filters used in digital relay protection current measuring elements. Mathematical descriptions of the considered digital filters as well as the computer programs for their coefficients calculation are described. It has been shown that in order to reliable estimate the digital filter performance its input signals waveforms must be close to the actual secondary current waveform of the current transformer to which the digital protection with the estimated digital filter is connected. For these purposes in MatLab–Simulink dynamic simulation environment the power system and the current measuring element models were developed. Performed calculations allowed to reveal that the exponentially decaying DC component which in some cases contains in primary fault current drives the current transformer core into saturation even when its nominal parameters are not exceeded. This results in distortion of the current transformer secondary current which in this case contains higher and inter-harmonics. Moreover, such harmonic content is not completely taking into account during coefficients calculation of the considered digital filters what results in signal magnitude estimation inaccuracy. Comparison of the digital filters response to the above-mentioned input signals allowed to find out such digital filter implementations which enable signal magnitude estimation with a minimum error. Ways of filtering quality improvement concerned with the window functions are proposed. Thus, the joint usage of digital filter and Hamming window allows to achieve the zero value of the signal magnitude gain factor in high-frequency range and substantially suppress all spectral components above 100 Hz. The increasing of the signal magnitude settling time in this case can be reduced by choosing the most optimal parameters of the all components of the current measuring element.
