    Full Text Available In Nabokov’s Lolita, Humbert Humbert’s The Enchanted Hunters, as a quest for love, aims to reconstruct a felicitous world or integrate various fragmentary details into an organic unity that revives a lost love, experiencing it on the basis of irony, and revealing a simulation of the desire, violence, and despondency which have been expressed in myths of nymphs and Persephone. The protagonist never reaches this unity, but his narrative of erotic and romantic love reveals him as a pathetic addict engaged in mechanical reproduction related to the phenomena of desire, seduction, violence, and sex. His The Enchanted Hunters does not simulate what he expects of his childhood love with Annabel; rather, it simulates the erotic imagination suggested in Mary D. Sheriff’s term “nymphomania,” in which artists fall degenerately to a model of tragedy. This seeming coincidence is actually not coincidental: Nabokov weaves a story concerning a pedophile’s seduction of a prepubescent child into a “story within a story,” in which the girl is imagined as a seducer who bewitches a number of hunters. Just as the girl in the play is a figment of a poet’s imagination, so Lolita in the novel Lolita is an imaginary production of a middle-aged pedophile. Yet Lolita is not so much a novel revealing guilt and mental disorder, but a mélange of art and reality, or more specifically, it is about a coinage in which the author fabricates art and myth in real life. Comment réconcilier l’art et la morale ? Dans la préface de Lolita, V. Nabokov pose, par l’entremise du préfacier fictif John Ray, un regard ambigu où le moralisme et le scientisme du clinicien s’entremêlent. Dans la postface rédigée par V. Nabokov, l’auteur revendique ouvertement ses choix esthétiques et affirme que Lolita n’est pas porté par une intention morale. La lecture détaillée de la postface révèle un auteur en butte au scandale qui éclate. Les choix poétiques sont donc explicités. La jouissance esthétique propre à l’art transcende ainsi le moralisme.     Full Text Available The present study is an attempt to use “textually oriented discourse analysis” of Norman Fairclough, (1941, to offer a new reading of Vladimir Nabokov’s Lolita (1955. The researcher found it quite appropriate to read the chosen work through Fairclough’s stylistic and discursive features in order to represent the identities of the main characters. Nabokov’s works represent an important portrayal of discursive challenges in the post-war American society that is largely invested in Fairclough’s theory. One of the most apparent aspects in Nabokov’s works is that of social identity and subject formation. As discourse theory denotes, a subject 'misrepresents' the world in ideology because he wants to do so, because there is some reward or benefit to him in doing so. Similarly Humbert Humbert in Lolita is looking for an imaginary world by which he can hide his suppressed identity. He fakes a new identity for himself through fiction. Considering the style of narration in Lolita, some reader consider Humbert as an unreliable narrator, he sometimes makes up some events and in some parts of the story he seems insane and uncertain. In Lolita Nabokov tries to show that each individual creates his/her reality and do not reflect reality. Humbert narrates his personal story from his point of view which might be different from people around him. Reality in Nabokov’s perspective is subjective and mixture of memory and imagination.

    Full Text Available In the afterword to Lolita, Nabokov claimed that in this book he had to invent both Lolita and America after having invented Europe in his previous fiction. This paper focuses precisely on the various ways in which Nabokov “invented” America in his best-known novel. This invention is first of all the result of the author’s evolving stance on the complexity of what he called “average ‘reality’” in his works. Through a survey of Nabokov’s statements on the choice and role of place in the forewords to his Russian works and in his critical texts, I show that Lolita is indeed considered by Nabokov to be a “recreation” of American reality, to a much greater extent than his Russian works had been recreations of a given milieu. I take the metaphor of the “crazy quilt” mentioned in Lolita to suggest complexity, chromatic exuberance, hybridity. The invention of America is also the result of a process of naming. Place names will be examined, not only those which make up Quilty’s “cryptogrammic paperchase”, but also Humbert’s choice of place names. The problem of referentiality is discussed and the way recent criticism has dealt with it. Finally, the interplay between one and many is emphasized, the way in which the diversity of the “crazy quilt” is counterbalanced by the uniqueness of the mastermind having produced it. The American motto “From many make one” could be reinterpreted as “From one make many”.

    Full Text Available The study aimed to reveal the social phenomenon in a literary work, especially pedophilia. The research used qualitative approach by applying library research using psychological theory introduced by Sigmund Freud. The data source was Lolita, an English novel, written by Vladimir Nabokov. The analysis is divided into three parts; they are the identification of main character, characterization, and pedophilia. The findings were as follows. First, the main character was Humbert as his high intensity in all the events that build the whole story. Second, the characterization described that the main character was obsessive, possessive, and immoral. Third, the role of literary work revealed pedophilia phenomenon. Finally, it can be concluded that the literary work has played a very important role in revealing the social phenomenon.

    Full Text Available Sézary syndrome (SS, an aggressive form of erythrodermic pruritic cutaneous T cell lymphoma (CTCL, from an immunological perspective characterized by increased Th2 cytokine levels, elevated serum IgE and impaired cellular immunity. Not only the clinical appearance but also the hallmark immunological characteristics of SS often share striking similarities with acute flares of atopic dermatitis (AD, a common benign chronic inflammatory skin disease. Given the overlap of several immunological features, the application of similar or even identical therapeutic approaches in certain stages of both diseases may come into consideration. The aim of this review is to compare currently accepted immunological aspects and possible therapeutic targets in AD and SS.

    Full Text Available Sézary syndrome (SS is a rare variant of primary cutaneous T-cell lymphoma. Little is known about the underlying pathogenesis of S. To address this issue, we used Affymetrix 10K SNP microarray to analyse 13 DNA samples isolated from 8 SS patients and qPCR with ABI TaqMan SNP genotyping assays for the validation of the SNP microarray results. In addition, we tested the impact of SNP loss of heterozygosity (LOH identified in SS cases on the gene expression profiles of SS cases detected with Affymetrix GeneChip U133A. The results showed: (1 frequent SNP copy number change and LOH involving 1, 2p, 3, 4q, 5q, 6, 7p, 8, 9, 10, 11, 12q, 13, 14, 16q, 17, and 20, (2 reduced SNP copy number at FAT gene (4q35 in 75% of SS cases, and (3 the separation of all SS cases from normal control samples by SNP LOH gene clusters at chromosome regions of 9q31q34, 10p11q26, and 13q11q12. These findings provide some intriguing information for our current understanding of the molecular pathogenesis of this tumour and suggest the possibility of presence of functional SNP LOH in SS tumour cells.

    Full Text Available The process of cultural globalisation does not always imply cultural homogenisation. Rather, it can be seen as a process of cultural ‘glocalisation’ and hybridisation where cultures continuously interact with and interpret each other to engender a hybrid cultural form. As Arjun Appadurai (1993 contends, neither centrality nor peripherality of culture exists in the context of cultural globalisation. Rather, transnational cultural forms are likely to circulate in multiple directions. This is particularly evident when examining Gothic & Lolita – a Japanese fashion trend which has emerged since the late 1990s. Applying Jan Nederveen Pieterse’s theory of ‘globalisation as hybridisation’ (2004, and Roland Robertson’s concept of ‘glocalisation’ (1995, this article attempts to explore how this fashion trend manifests the process of cultural ‘glocalisation’, hybridisation, and interaction through the localisation of Western Goth subculture and especially, historical European dress styles in Japan. In addition, it explores how the fashion signifies the idea of ‘reverse’ flow of culture through an ethnographic observation of an English-speaking online community devoted to the trend. The analysis of the online community also seeks to establish the idea that this transnational cultural flow serves as an alternative to the local culture.

    Angiogenesis is an important step to support progression of malignancies, including mycosis fungoides (MF) and Sézary syndrome (SS). Vascular endothelial growth factor (VEGF)-A, a key player in angiogenesis, is secreted by tumor cells of MF/SS and its expression levels in lesional skin correlated with disease severity. In this study, we examined serum VEGF-A levels in MF/SS patients. Serum VEGF-A levels were elevated in patients with erythrodermic MF/SS and the levels decreased after treatment. Importantly, serum VEGF-A levels positively correlated with markers for pruritus. We also found that VEGF-A upregulated mRNA expression of thymic stromal lymphopoietin by keratinocytes. Taken together, our study suggests that VEGF-A can promote progression and pruritus in MF/SS. Inhibition of VEGF-A signaling can be a therapeutic strategy for patients with erythrodermic MF/SS. © 2017 Japanese Dermatological Association.

    We have analyzed the outcome of mycosis fungoides (MF) and Sézary syndrome (SS) patients using the recent International Society for Cutaneous Lymphomas (ISCL)/European Organisation for Research and Treatment of Cancer (EORTC) revised staging proposal. Overall survival (OS), disease-specific survival (DSS), and risk of disease progression (RDP) were calculated for a cohort of 1,502 patients using univariate and multivariate models. The mean age at diagnosis was 54 years, and 71% of patients presented with early-stage disease. Disease progression occurred in 34%, and 26% of patients died due to MF/SS. A significant difference in survival and progression was noted for patients with early-stage disease having patches alone (T1a/T2a) compared with those having patches and plaques (T1b/T2b). Univariate analysis established that (1) advanced skin and overall clinical stage, increased age, male sex, increased lactate dehydrogenase (LDH), and large-cell transformation were associated with reduced survival and increased RDP; (2) hypopigmented MF, MF with lymphomatoid papulosis, and poikilodermatous MF were associated with improved survival and reduced RDP; and (3) folliculotropic MF was associated with an increased RDP. Multivariate analysis established that (1) advanced skin (T) stage, the presence in peripheral blood of the tumor clone without Sézary cells (B0b), increased LDH, and folliculotropic MF were independent predictors of poor survival and increased RDP; (2) large-cell transformation and tumor distribution were independent predictors of increased RDP only; and (3) N, M, and B stages; age; male sex; and poikilodermatous MF were only significant for survival. This study has validated the recently proposed ISCL/EORTC staging system and identified new prognostic factors.

    zary syndrome (Sz) is a malignancy of CD4+ memory skin-homing T cells and presents with erythroderma, lymphadenopathy, and peripheral blood involvement. To gain more insight into the molecular features of Sz, oligonucleotide array analysis was performed comparing gene expression patterns of CD4+ T cells from peripheral blood of patients with Sz with those of patients with erythroderma secondary to dermatitis and healthy controls. Using unsupervised hierarchical clustering gene, expression patterns of T cells from patients with Sz were classified separately from those of benign T cells. One hundred twenty-three genes were identified as significantly differentially expressed and had an average fold change exceeding 2. T cells from patients with Sz demonstrated decreased expression of the following hematopoietic malignancy-linked tumor suppressor genes: TGF-beta receptor II, Mxi1, Riz1, CREB-binding protein, BCL11a, STAT4, and Forkhead Box O1A. Moreover, the tyrosine kinase receptor EphA4 and the potentially oncogenic transcription factor Twist were highly and selectively expressed in T cells of patients with Sz. High expression of EphA4 and Twist was also observed in lesional skin biopsy specimens of a subset of patients with cutaneous T cell lymphomas related to Sz, whereas their expression was nearly undetectable in benign T cells or in skin lesions of patients with inflammatory dermatoses. Detection of EphA4 and Twist may be used in the molecular diagnosis of Sz and related cutaneous T-cell lymphomas. Furthermore, the membrane-bound EphA4 receptor may serve as a target for directed therapeutic intervention.

    zary syndrome (SzS) is an advanced form of cutaneous T-cell lymphoma characterized by peripheral blood involvement, impaired cell-mediated immunity, and T-helper 1 (TH1) cytokine production. To understand the mechanism of these defects, we studied the expression and function of CD40L in peripheral blood mononuclear cells (PBMCs) of patients with SzS. We found that PBMCs of patients with SzS have a defect in interleukin-12 (IL-12) and tumor necrosis factor-alpha (TNF-alpha) production upon anti-CD3 stimulation and that tumor CD4+ T lymphocytes have a specific defect in CD40L induction after anti-CD3 ligation in vitro. This defect may explain the poor IL-12 production, because IL-12 production by anti-CD3-stimulated PBMCs was dependent on CD40L in healthy donors. The observed defect in tumor cell CD40L expression appears to be due to inappropriate T-cell signaling upon CD3 ligation, because expression of other T-cell activation antigens such as CD25, and to a lesser extent CD69, are also impaired on tumor cells. Importantly however, the inability of SzS PBMCs to appropriately produce IL-12 and TNF-alpha could be restored by recombinant hexameric CD40L. Taken together, our results demonstrate that impaired IL-12 and TNF-alpha production in SzS is associated with defective CD4+ T lymphocyte CD40L induction and indicate that CD40L may have therapeutic potential in SzS.

    Monoclonal antibodies (MAbs) reacting with the human T cell receptor (TCR) V beta or V alpha region have been shown to be almost as specific as a private idiotypic MAb in identifying T cell clones. When available, V beta-specific MAbs offer the ease of immunofluorescence analysis to identify and quantitate expanded malignant or nonmalignant T cell populations without requiring polymerase chain reaction (PCR) technology to evaluate expression of V beta gene families. The V beta expression of peripheral blood lymphocytes from twenty-three consecutive patients with Sézary syndrome has been analyzed by reverse transcriptase (RT)-PCR. Ten patients had malignant T cell clones that expressed a TCR V beta corresponding to a commercially available anti-V beta antibody. Immunofluorescence staining with anti-V beta MAbs showed a direct correlation with RT-PCR results in seven of ten patients. No false positive reactivity was noted on immunofluorescence staining with any MAb. Cells from three patients, however, did not react with the corresponding anti-V beta MAb. These three cases expressed a TCR V beta from gene families containing a single member, ie, V beta 14, V beta 18, and V beta 20, yet MAbs reported to be specific for these regions failed to react with the T cell clone from these patients. Sequencing of the PCR product in these cases confirmed the RT-PCR results. Cells from two patients expressed a TCR using V beta 5.1-D beta 1.1 genes with different J-C segments. One patient's cells reacted with an anti-V beta 5.1 MAb (LC4) whereas the other patient's cells bound one-tenth the amount of this same MAb. These results indicate that currently available anti-TCR V region MAbs may not react consistently with T cell clones expressing the corresponding V region or may react with a low affinity making detection difficult.     As with many immunopathologically driven diseases, the malignant T cells of cutaneous T-cell lymphomas (CTCLs), such as Sézary syndrome, display aberrant cytokine secretion patterns that contribute to pathology and disease progression. Targeting this disordered release of cytokines is complicated by the changing cytokine milieu that drives the phenotypic changes of CTCLs. Here, we characterize a novel signaling pathway that can be targeted to inhibit the secretion of cytokines by modulating either CXCR4 or CXCR4-mediated signaling. We demonstrate that upon ligation of the T-cell antigen receptor (TCR), the TCR associates with and transactivates CXCR4 via phosphorylation of S339-CXCR4 in order to activate a PREX1-Rac1-signaling pathway that stabilizes interleukin-2 (IL-2) , IL-4 , and IL-10 messenger RNA (mRNA) transcripts. Pharmacologic inhibition of either TCR-CXCR4 complex formation or PREX1-Rac1 signaling in primary human T cells decreased mRNA stability and inhibited secretion of IL-2, IL-4, and IL-10. Applying this knowledge to Sézary syndrome, we demonstrate that targeting various aspects of this signaling pathway blocks both TCR-dependent and TCR-independent cytokine secretion from a Sézary syndrome-derived cell line and patient isolates. Together, these results identify multiple aspects of a novel TCR-CXCR4-signaling pathway that could be targeted to inhibit the aberrant cytokine secretion that drives the immunopathogenesis of Sézary syndrome and other immunopathological diseases. © 2017 by The American Society of Hematology.

    This pathology PILOT study aims to define the role and feasibility of centralized pathology review in a cohort of 75 patients from different centers in the United States and Europe using digital slide scanning. The pathologic material from 75 patients who had been diagnosed with mycosis fungoides/Sézary syndrome and were clinically staged as IIb or above was retrieved from 11 participating centers. Each pathology reviewer was provided with the pathologic diagnosis (by the referring pathologist), and the following list of histopathologic criteria (presence or absence) from the initial report: epidermotropism, folliculotropism (FT), large cell transformation, syringotropism, and granulomas. Patients with advance stage were selected for this study as this is a population where there is significant variability in the diagnosis of pathologic prognostic and predictive biomarkers. The slides were digitally scanned with an Aperio scanner and consensus review of cases occurred when major or minor discrepancies between the referral diagnosis and central pathology review occurred. Among the 75 cases, 70 (93.3%) had a final consensus diagnosis between the 3 central review pathologists. The overall agreement between the consensus review and the referring pathologist was 60%. The overall agreement was also higher between the reviewers and consensus review, compared with the referring pathologist and consensus. 65.3% of cases had some type of discrepancy (major or minor) between the outside and consensus review. Major discrepancies were seen in 34 of 73 cases (46.6%; 73 cases indicated a yes or no response). Minor discrepancies were seen in 32 of 75 (42.7%) of cases. Most of the major discrepancies were accounted by a difference in interpretation in the presence or absence of large cell transformation or FT. Most minor discrepancies were explained by a different interpretation in the expression of CD30. We found digital slide scanning to be a beneficial, reliable, and practical

Materials and methods: A total of 18 cases with non-MF/SS CML, confirmed by pathology or on clinical grounds, were analyzed in this study. The sensitivity of CT and PET/CT scans in the diagnosis of primary skin lesions, as well as the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of CT and PET/CT scans in the diagnosis of lymph nodes (LNs) and other organs (except skin and LNs) were calculated. Results: The diagnostic sensitivity of CT and PET/CT scans in the diagnosis of primary skin lesions was 82.4% (14/17) and 100% (17/17), respectively. The diagnostic sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of CT and PET/CT scans in the diagnosis of LN lesions were 55.6% (5/9), 88.9% (8/9), 72.2% (13/18), 83.3% (5/6), 66.7% (8/12), and 88.9% (8/9), 100% (9/9), 94.4% (17/18), 100% (8/8), 90.0% (9/10), respectively. The diagnostic value of the CT and PET/CT scans in the diagnosis of involvement of other organs, were 40.4% (2/5), 100% (13/13), 83.3 (15/18), 100% (2/2), 81.3% (13/16) and 80.6% (4/5), 100% (13/13), 94.4% (17/18), 100% (3/3), 92.9% (13/14), respectively.     Purpose Advanced-stage mycosis fungoides (MF; stage IIB to IV) and Sézary syndrome (SS) are aggressive lymphomas with a median survival of 1 to 5 years. Clinical management is stage based; however, there is wide range of outcome within stages. Published prognostic studies in MF/SS have been single-center trials. Because of the rarity of MF/SS, only a large collaboration would power a study to identify independent prognostic markers. Patients and Methods Literature review identified the following 10 candidate markers: stage, age, sex, cutaneous histologic features of folliculotropism, CD30 positivity, proliferation index, large-cell transformation, WBC/lymphocyte count, serum lactate dehydrogenase, and identical T-cell clone in blood and skin. Data were collected at specialist centers on patients diagnosed with advanced-stage MF/SS from 2007. Each parameter recorded at diagnosis was tested against overall survival (OS). Results Staging data on 1,275 patients with advanced MF/SS from 29 international sites were included for survival analysis. The median OS was 63 months, with 2- and 5-year survival rates of 77% and 52%, respectively. The median OS for patients with stage IIB disease was 68 months, but patients diagnosed with stage III disease had slightly improved survival compared with patients with stage IIB, although patients diagnosed with stage IV disease had significantly worse survival (48 months for stage IVA and 33 months for stage IVB). Of the 10 variables tested, four (stage IV, age > 60 years, large-cell transformation, and increased lactate dehydrogenase) were independent prognostic markers for a worse survival. Combining these four factors in a prognostic index model identified the following three risk groups across stages with significantly different 5-year survival rates: low risk (68%), intermediate risk (44%), and high risk (28%). These findings are relevant as around 20% of young people are affected by moderate-to-severe acne, and indeed acne severity is associated with increased risks of anxiety, depression and suicidal ideation.2 The authors identified that “stress severity strongly correlated with an increase in acne severity,” and suggested that stress likely has an important role in the pathogenesis of acne.1 However, the paper could have been taken further to better establish the true extent of this relationship, and ultimately whether certain patients will benefit from clinical interventions based on their perceived stress scale. View the original paper by Zari and Alrahmani.

    Full Text Available Chronic myelopathy associated with T-lymphotropic virus type I (HAM has been described as an endemic disease in several areas of the world, meanwhile there are few papers describing the association between HAM and adult T cell leukemia-lymphoma. We report the case of a man that, after four years of progressive spastic paraparesis and neurogenic bladder, developed a clinical picture of a lymphoproliferative disorder characterized by dermal and systemic envolvement, mimicking mycosis fungoides/Sézary syndrome.Apesar da infecção pelo HTLV-I ser endêmica em várias regiões do mundo, poucos são os relatos da associação entre leucemia-linfoma de células T do adulto (ATLL e encefalomieloneuropatia pelo HTLV-I. No presente artigo é descrito um paciente que no curso do comprometimento neurológico pelo HTLV-I desenvolveu quadro de leucemia com infiltração de tecido dérmico semelhante ao encontrado na micose fungóide/síndrome de Sézary.

    Full Text Available The Monte Carlo method (also known as the Monte Carlo simulation was proposed by Nicholas Metropolis, S. Ulam and Jhon Von Neiman in 50-th years of the past century. The method can be widely applied to analysis of investment projects due to the advantages recognized both by practitioners and the academic community. The balance model of a project with discounted financial flows has been implemented for Microsoft Excel and Google Docs spread-sheet solutions. The Monte Carlo method for project with low and high correlated net present value (NPV parameters in the environment of the electronic tables of MS Excel/Google Docs. A distinct graduation of risk was identified. A necessity of account of correlation effects and the use of multivariate imitation during the project selection has been demonstrated.

    An investigation is made on the behaviour of Cu2 + and Fe3 + in copper doped myoglobin, subjected to heat treatment. ESR is observed at X-band. The amplitude of the g = 5.9 line of the high spin F 3 + in met-myoglobin is studied as a function of the temperature of the heat treatment, the pH and the length of time of exposure to the heat treatment. These experiments are performed for both the pure protein and for the copper-myoglobin complex, at pH between 5 and 8.5. Results concerning the decrease of the amplitude of Fe 3+ high spin signal are discussed in comparison with Hollocher's results in hemoglobin. The Cu spectra obtained are interpreted, with the aid of a computer program, Kivelson and Neiman's results being used to analyse the authors data

    Full Text Available Cutaneous T cell lymphomas (CTCLs are a heterogeneous group of extranodal non-Hodgkin’s lymphomas that are characterized by a cutaneous infiltration of malignant monoclonal T lymphocytes. They typically afflict adults with a median age of 55 to 60 years, and the annual incidence is about 0.5 per 100,000. Mycosis fungoides, Sézary syndrome, and primary cutaneous peripheral T cell lymphomas not otherwise specified are the most important subtypes of CTCL. CTCL is a complicated concept in terms of etiopathogenesis, diagnosis, therapy, and prognosis. Herein, we summarize advances which have been achieved in these fields.

Os linfomas cutâneos de células T/NK de comportamento clínico indolente compreendem a micose fungóide clássica, a micose fungóide foliculotrópica, a reticulose pagetóide, a cútis laxa granulomatosa, o linfoma cutâneo primário de grande célula anaplásica, a papulose linfomatóide, o linfoma subcutâneo de célula T paniculite-símile e o linfoma cutâneo primário de pequena e média célula T CD4+ pleomórfica. Os linfomas cutâneos de células T/NK de comportamento agressivo incluem a síndrome de Sézary, o linfoma extranodal de célula T/NK, tipo nasal, o linfoma cutâneo primário agressivo de célula T CD8+ epidermotrópica, o linfoma cutâneo de célula T gd e o linfoma cutâneo primário de célula T periférica, não especificado.     Full Text Available Sophia Rangwala, Madeleine Duvic, Chunlei ZhangDepartment of Dermatology, The University of Texas MD Anderson Cancer Center, Houston, Texas, USAAbstract: Epigenetic modification with small molecule histone deacetylase inhibitors has been a promising new anti-neoplastic approach for various solid and hematological malignancies, particularly cutaneous T-cell lymphoma (CTCL. Oral vorinostat was the first histone deacetylase inhibitor approved to enter the clinical oncology market for treating CTCL patients who have progressive, persistent, or recurrent disease after failing two systemic therapies. In two phase II clinical trials, oral vorinostat was found to be safe and effective at a dose of 400 mg/day, with an overall response rate of 24%–30% in heavily pretreated patients with advanced CTCL, including those with large-cell transformed mycosis fungoides and Sézary syndrome. About half of CTCL patients receiving vorinostat also experienced substantial relief in pruritus and thus a marked improvement in quality of life. A subsequent follow-up study reported long-term safety and clinical benefits of vorinostat in patients with refractory CTCL, regardless of previous treatment failures. The most frequent side effects of vorinostat include gastrointestinal symptoms, fatigue, and thrombocytopenia. These adverse reactions are dose-related and reversible upon cessation of therapy. Preclinical studies have supported the therapeutic potential of vorinostat by demonstrating in vitro and in vivo anti-tumor activities against CTCL, including selective induction of apoptosis in malignant T cells, inhibition of angiogenesis, suppression of signal transducer and activator of transcription proteins, and up-regulation of pro-apoptotic proteins. Identification of biomarkers of response and resistance will help select CTCL patients most likely to benefit from treatment and guide the design of effective combination therapies.Keywords: cutaneous T-cell lymphoma

    Full Text Available Fotoferese extracorpórea é uma modalidade de terapia celular que tem demonstrado eficácia e segurança em pacientes portadores de doenças auto-imunes e mediadas por células T, incluindo esclerodermia, linfoma de células T cutâneo/ Síndrome de Sézary e doença enxerto-contra-hospedeiro após transplante de células-tronco hematopoiéticas. Tem sido empregada, ainda, com o intuito de induzir tolerância em transplantes de órgãos sólidos. O tratamento se baseia no efeito biológico de uma substância fotossensibilizante, o 8-metoxipsoraleno, e da radiação ultravioleta A, nas células mononucleares coletadas por aférese e reinfundidas ao paciente. O mecanismo de ação parece ser devido à indução de imunidade anticlonotípica direcionada a clones patogênicos de células T. O tratamento induz a apoptose de células T patogênicas, e a ativação de células apresentadoras de antígenos desempenha papel importante no processo imunomodulador.Extracorporeal photopheresis is a method of cell therapy that has demonstrated efficacy in autoimmune diseases, cutaneous T-cell lymphoma/Sézary syndrome and graft-versus-host disease after hematopoietic stem-cell transplantation. This technique has also been employed as a treatment strategy to induce tolerance after solid organ transplants. Treatment is based on the biological effect of ultraviolet A radiation and a photosensitizing drug, 8-methoxypsoralen, on mononuclear cells collected by apheresis and reinfused into the patient. The suggested mechanism of action is related to the induction of anticlonotypic immunity directed against pathogenic clones of T lymphocytes. Treatment induces apoptosis of pathogenic T-cells, and it appears that activation of antigen-presenting cells has an important role in this process.

    Uranium mineralization occurring in the Triassic sediments in the Polish area is discussed. Systematic work conducted for over ten years revealed the presence of uranium mineralization in the following geological units: Peribaltic syneclize, Fore-Sudetic monocline, Zary pericline and Pomerania trough. Out of three uranium-bearing horizons which can be correlated with each other two (the lower and the middle) are connected with the Middle Bunter, while the upper horizon is related to the Upper Bunter. Mineralization was found in sandstones, conglomerates, mudstones and claystones and, in the Fore-Sudetic monocline, also in carbonates. Among uranium minerals uranium black and coffinite were identified. In addition to uranium, increased vanadium, selenium and molybdenum contents were found in the sandstones. Some of the uranium-bearing horizons are of economic value. (author)

    Full Text Available In this paper, the author argues for a theoretical difference between the modern and postmodern aestheticism and considers their political implications arguing that modern aestheticism carried progressive or radical political impulse; whereas, under postmodernism, its character shifted into a reactionary politics. The specific political implications that are derived from these two kinds of aestheticism concern the attitude towards human nature, different kinds of narcissistic tendencies and divergent attitudes towards art/culture. Next, the author uses the examples of Oscar Wilde's Picture of Dorian Gray and Nabokov's Lolita to demonstrate how these differences manifest themselves in the works of these two prominent, if not pivotal, artists related to the aesthetic movement in the history of literature. It is concluded that the politics of Oscar Wilde and Vladimir Nabokov differ radically and these differences stem from the fact that their works embody divergent conceptions of aestheticism.

    Full Text Available This study focuses on three recent literary works (fiction and non-fiction novels written by authors of Asiatic origins: the French-speaking Chinese writer, Dai Sijie, the Iranian professor, Azar Nafisi, and the Japanese writer, Murakami Haruki. In the titles of these international bestsellers (Balzac and the Little Chinese Seamstress, Reading Lolita in Tehran, and Kafka on the Shore, we find an oxymoron: the name of a famous, canonical Western writer or character is used in an unusual context, linked to the geography of the near or far East. The consequent effect of estrangement can restore strength to literature, perhaps with greater determination than is possible in Western cultures. By using (and reversing the coordinates of Edward Said’s idea of Orientalism and, as in Kafka on the Shore, certain concepts of the Lacanian theory, this study analyzes the transfer of the European literary myths towards Eastern cultures.

    Full Text Available Celem artykułu jest zaproponowanie zarysu autorskiej koncepcji utylitarystycznej etyki controllingowego pomiaru dokonań. Celami szczegółowymi są: wyjaśnienie pojęcia etyki oraz przedstawienie sposobów jej rozumienia, wyjaśnienie założeń paradygmatu utylitarystycznego w etyce i określenie założeń etyki utylitarnej, zdefiniowanie specyfiki controllingowego pomiaru dokonań oraz wypracowanie do zastosowania w odniesieniu do tego ostatniego założeń etyki w paradygmacie utylitarystycznym. Podstawą do przedstawionych rozważań będą interdyscyplinarne studia literaturowe obejmujące filozofię, etykę, zarządzanie, rachunkowość i controlling.   1. [Cytostructure of the mantle zone in lymphatic tissue].

    Four cellular layers of the follicular mantle zone in palatine tonsil lymphatic tissue were studied by electron microscopy after simultaneous immunophenotypical investigation. The first layer of the mantle zone consisting of small blastic cells was analogous to the small (centrocytoid) blastic layer of germinal centres. The second B monocytoid layer was lacking analogy in basic series of lymphocytes and seemed to be an independent morphological and probably functional unit. Plasmacytoid and clarocellular elements in outer layers of follicular mantle zone were in a way similar to T plasmacytoid and clarocellular components of Sézary syndrome infiltrates but considering transitional forms they had a local origin from incompletely transformed elements of B monocytoid layer. Inner follicular mantle zone was discussed as a source of incompletely transformed B lymphocytes for further mantle layers where their immunophenotypical modulation is taking place according to actual need. Outer mantle layers are aggressive against damaged epithelial and litoral structures and may be instrumental in a common reaction of B and T components.

    Full Text Available [An outline of the literature of the last sixteen years] written by Piotr Chmielowski was designed in the author’s intention as a sketch of “rather journalistic than historical features”. The above is clearly indicated by the forewords written by the author to its successive four editions published between 1881 and 1898. Still, despite its lack of pretentions, the book acquired with time the status of a fully-fledged instructive approach to Polish literature of the Positivist period. A closer look at the text with regard to the proposed periodization reveals, however, a number of flaws and aporias committed by Chmielowski, particularly in establishing boundary dates of the proposed literary period. The present article attempts to prove that the source for the inaccuracies and inconsistencies of the solutions proposed in Zarys is not so much in their makeshift or the pro tem nature, but rather in the aggressively promoted ideology, represented by Chmielowski, that completely disregarded the nature of literary phenomena.

    The T alpha and T beta chains of the heterodimeric T-lymphocyte antigen receptor are encoded by separated DNA segments that recombine during T-cell development. We have used rearrangements of the T beta gene as a widely applicable marker of clonality in the T-cell lineage. We show that the T beta genes are used in both the T8 and T4 subpopulations of normal T cells and that Sézary leukemia, adult T-cell leukemia, and the non-B-lineage acute lymphoblastic leukemias are clonal expansions of T cells. Furthermore, circulating T cells from a patient with the T8-cell-predominantly lymphocytosis associated with granulocytopenia are shown to be monoclonal. Finally, the sensitivity and specificity of this tumor-associated marker have been exploited to monitor the therapy of a patient with adult T-cell leukemia. These unique DNA rearrangements provide insights into the cellular origin, clonality, and natural history of T-cell neoplasia.

    According to the latest update, 2578 unique mature miRNAs are currently annotated in the human genome and participate in the regulation of multiple events, such as cellular proliferation or apoptosis. A previous study analyzing global miRNA expression patterns in GH cells (high HERV-K versus low) showed that two miRNAs (miR-663 and miR-638) are differentially regulated and exhibit expression parallel to that of HERV-K. The aim of this study was to evaluate HERV-K and -W pol gene and mir-155 expression in SS patients and possible relationship between them. The comparison between SS patients and healthy donor showed a significant difference in terms of mir-155 expression p=0.0003 as previously reported by our groups. We demonstrated that HERV-K and -W pol gene expression was significantly higher in SS patients vs healthy donor as previously reported by our groups. Our correlation data suggest that miR-155 are not directly involved in regulating the HERVs. Furthermore, further studies including other cohorts of pathology with mir-155 and HERVs involvement such as inflammatory diseases are needed to investigate the role of mir-155 in the cross-activations of HERVs.

    Full Text Available Both in Speak, Memory and in Strong Opinions, Nabokov insists on his early proficiency in English, French. This authorial stance makes it easy to believe that the writer’s transition to English was easy. And yet, Nabokov’s correspondence with publishers and his literary agent, Altagracia de Jannelli, reveals that this conversion was torturous and required extensive support from native speaker editors and translators. The essay documents Nabokov’s inner turmoil at the time when he began to explore the British and American markets. In spite of the publication of Camera Obscura in England (1936 and, as Laughter in the Dark, in the US (1938, his other works’ journeys to the Anglophone reader required time and effort. A close reading of the famous afterword to Lolita, a comparative analysis of Winifred Roy’s translation of Camera Obscura and Nabokov’s self-translation of Laughter in the Dark, and the perusal of the author’s correspondence illustrate the difficulties he had to overcome in order to convey stylistic intricacy of his fiction to this new audience.

    Full Text Available Background: There have been controversial reports about the possible association between mycosis fungoides (MF, its leukemic variant Sιzary syndrome (SS and human T lymphotropic virus type 1 (HTLV-1 in different geographical regions. Aims: The purpose of this study was to explore any association between MF and presence of HTLV-1 infection in Iran. Methods: In a case-control setting, 150 clinically and histopathologically proven MF patients had been admitted to the tertiary referral skin center during a 10-year period and another 150 normal volunteers had been compared with each other for the presence of HTLV-1 infection. Enzyme-linked immunosorbent assay (ELISA was used to detect antibodies against HTLV-1, and positive results were confirmed with western blotting. Results: Only three MF patients had HTLV-1 infection, whereas two cases of normal subjects had the infection ( P > 0.05. The only three seropositive MF patients were male and from North-Eastern Iran . Conclusion: This study showed that MF does not correlate with HTLV-1 infection in Iran.

    Full Text Available The paper deals with the relation of power established by narrative texts, exploring the many-sided field of articulation among the related subjects: the narrator as a subject of power, the text as a rhetorical device, and the reader as an ultimate guarantor of meaning. After a brief introduction on the “rebirth of rhetoric” in the second half of the Twentieth century, drawing attention to its links with Reader-response criticism, the paper  focuses on the “power of words” and analyzes three case-studies: Lolita by Vladimir Nabokov (1955, Money by Martin Amis (1984 and The Kindly Ones by Jonathan Littell (2006. These novels depend for their rhetorical effect on the invention of a special narrating voice and on the relationship that it establishes with the reader – an odd mixture of antagonism and complicity, seduction and persuasion. The reader is thus invoked as a brother, but an ambiguous and untrustworthy one, as archetypally described in last verse of the opening poem from Baudelaire’s Les Fleurs du mal: «hypocrite lecteur – mon semblable, mon frère».

    Full Text Available The aim of this article is to demonstrate close links between 'experience' as an epistemological notion and 'femininity' as a gender concept. Experience is considered here within two dichotomies: experience - reason; and experience - innocence. They are traced here mainly in the writings of Rousseau, Wollstonecraft, Krasicki, Mickiewicz, Blake and American Transcendentalists. Both dichotomies constitute an essential part of the Enlightenment and Romanticism thought, art and lifestyle. In particular they construct the code of sensibility, which highly influenced the 18lh and 19,h centuries aesthetics, spirituality and philosophy. As a result of the cult of sensibility in Western culture there appeared a peculiarly gendered figurę - that of a sensitive man, a feminized małe, capable of experiencing and showing strong emotions. By connecting maleness with features usually perceived as typically feminine, the latter gained higher cultural status. The counterpart for the sensitive man was a reasonable woman, a female person endowed with traditionally masculine intellectual capacities. The discourse of experience has a special meaning in William Blake's work and thought; here it is related to his anti-rationalist and antiscientific worldview, as well as to his apotheosis of Innocence as the State of elear insight into the true reality - one that is hidden behind the visible world. From Blake, via European Romanticists and American Transcendentalists it is possible to draw links to contemporary counterculture, environmental movements and theories (eco-criticism, eco-feminism with their appreciation of soft feminine values.

    Full Text Available FUNDAMENTOS - A dermatite esfoliativa/eritrodermia é síndrome rara e grave, de etiologia freqüentemente desconhecida. A hospitalização é necessária para avaliação inicial e tratamento. OBJETIVO - Determinar o perfil clínico, laboratorial e histopatológico dos eritrodérmicos internados no HUCFF-UFRJ. MÉTODOS - No período de 1990 a 2007, 170 casos foram submetidos a hemograma, velocidade de hemossedimentação, bioquímica do sangue, provas de função hepática, proteínas totais e frações, pesquisa de células de Sézary no esfregaço de sangue periférico, radiografia de campos pleuropulmonares e biópsia de pele em três locais. RESULTADOS - Entre 92 homens e 78 mulheres, na faixa de 30 a 80 anos, identificaram-se 99 casos (58,23% com dermatoses preexistentes; 37 (21,77% com farmacodermias; 18 (10,58% com linfomas cutâneos de células T e 16 (9,4% de causa indeterminada. Calafrios, prurido, linfonodomegalias e edema de membros inferiores destacaram-se dentre sinais e sintomas. O perfil laboratorial consistiu de aumento da VHS, eosinofilia, leucocitose, linfocitose e anemia. No grupo de etiologia indeterminada, dois padrões histológicos emergiram: dermatite psoriasiforme e dermatite inespecífica.     Cutaneous T-cell lymphomas (CTCLs) are dominated by mycosis fungoides (MF) and Sézary syndrome (SS), and durable disease control is a therapeutic challenge. Standard total skin electron beam therapy (TSEBT) is an effective skin-directed therapy, but the possibility of retreatments is limited to 2 to 3 courses in a lifetime due to skin toxicity. This study aimed to determine the clinical effect of low-dose TSEBT in patients with MF and SS. In an open clinical study, 21 patients with MF/SS stages IB to IV were treated with low-dose TSEBT over dose of 10 Gy in 10 fractions. Data from 10 of these patients were published previously but were included in the current pooled data analysis. Outcome measures were response rate, duration of response, and toxicity. The overall response rate was 95% with a complete cutaneous response or a very good partial response rate (dose (10-Gy) TSEBT offers a high overall response rate and is relatively safe. With this approach, reirradiation at times of relapse or progression is likely to be less toxic than standard dose TSEBT. It remains to be established whether adjuvant and combination treatments can prolong the beneficial effects of low-dose TSEBT. Copyright © 2015 Elsevier Inc. All rights reserved.

W celu opisu, w jaki sposób marksizm podchodzi do kwestii postkapitalistycznej rzeczywistości, bazowałem głównie na pismach Karola Marksa, Fryderyka Engelsa, Lwa Trockiego i Ernesta Mandela, w oparciu o które próbowałem stworzyć zarys modelu społeczno-gospodarczego. Opis, jaki wyłania się z prac wymienionych autorów, przedstawia społeczeństwo oparte na demokracji robotniczej w miejscu pracy (aspekt ekonomiczny i na poziomie państwa (aspekt polityczny oraz planowaniu demokratycznemu, mającemu na celu sukcesywne zaspokajanie oraz bardziej złożonych potrzeb społecznych. Tak opisany model postkapitalistycznego społeczeństwa stanowi radykalnie zaprzeczenie rzeczywistości, jaka istniała w (poststalinowskich reżimach biurokratycznych.

    Numerous researchers have been studying the liquid-phase autoxidation by molecular oxygen of compounds containing a mercapto group. It has been stated that, in the course of the reaction, sulphide is formed, but the exact mechanism of this process has not been determined. The oxidation of mercaptoacetic acid, at 50{sup o}C and at a pH of 8, was investigated both in the presence of Complexon IV and in the presence of iron-ions. Na{sub 2}S, labelled with S{sup 35}, was introduced into the system and it was determined that this sulphide considerably increases the rate of the oxygen uptake both in the processes catalysed by trivalent iron and free of iron-ions. In the course of the reaction the concentration of the sulphide, its specific activity and its total activity were measured. In using these data and on the basis of the kinetic isotopic method developed by Neiman and co-workers, it was possible to calculate the rate of sulphide formation and its further oxidation. In comparing these data with other kinetic data, it is shown that the oxidation of mercaptoacetic acid in the liquid phase is a branching chain reaction; one of its principal chains being the formation of Na{sub 2}S and its further oxidation. Moreover, it was determined that sulphate is formed in the course of the reaction only through the inorganic sulphide. Iron-ions catalyse the formation of organic disulphide and therefore also have influence on the sulphide formation.     Restimulation of previously activated T cells via the T-cell receptor (TCR) leads to activation-induced cell death (AICD), which is, at least in part, dependent on the death receptor CD95 (APO-1, FAS) and its natural ligand (CD95L). Here, we characterize cutaneous T-cell lymphoma (CTCL) cells (CTCL tumor cell lines and primary CTCL tumor cells from CTCL patients) as AICD resistant. We show that CTCL cells have elevated levels of the CD95-inhibitory protein cFLIP. However, cFLIP is not responsible for CTCL AICD resistance. Instead, our data suggest that reduced TCR-proximal signaling in CTCL cells is responsible for the observed AICD resistance. CTCL cells exhibit no PLC-gamma1 activity, resulting in an impaired Ca(2+)release and reduced generation of reactive oxygen species upon TCR stimulation. Ca(2+) and ROS production are crucial for up-regulation of CD95L and reconstitution of both signals resulted in AICD sensitivity of CTCL cells. In accordance with these data, CTCL tumor cells from patients with Sézary syndrome do not up-regulate CD95L upon TCR-stimulation and are therefore resistant to AICD. These results show a novel mechanism of AICD resistance in CTCL that could have future therapeutic implications to overcome apoptosis resistance in CTCL patients.

    Full Text Available Neyshabour turquoise mine is located in northwest of Neyshabour, southern Quchan volcanic belt. Eocene andesite and dacite forming as lava and pyroclastic rocks cover most of the area. Subvolcanic diorite to syenite porphyry (granitoids of magnetite series intruded the volcanic rocks. Both volcanic and subvolcanic rocks are highly altered. Four types of alteration are recognized including: silicification, argillic, calcification and propylitic. Silicification is dominant followed by argillic alteration. Mineralization is present as stockwork, disseminated and hydrothermal breccia. Hypogene minerals are pyrite, magnetite, specularite, chalcopyrite, and bornite. Secondary minerals are turquoise, chalcocite, covellite, and iron oxides. A broad zone of gossan has developed in the area. Oxidized zone has a thickness of about 80 m. Mineralized samples show high anomalies of Cu, Au, Zn, As, Mo, Co, U, LREE, Nb, and Th. Both aeromagnetic and radiometric (U and Th maps show very strong anomalies (10 × 5km within the mineralized area. Based on geology, alteration, mineralization, geochemistry, and geophysics, Neyshabour turquoise mine is a large Iron oxide Cu-Au-U-LREE (IOCG mineralized system. In comparison with other IOCG deposits, it has some similarities with Olympic Dam (Australia and Candelaria (Chile. In comparison with Qaleh Zari and Kuh Zar mines, Neyshabour turquoise mine is the first Iron oxide Cu-Au-U-LREE (IOCG mineralized system discovered in Iran.

    Mycosis fungoides (MF) is an uncommon cutaneous neoplasm in dogs. Treatment options are limited. Total skin electron therapy (TSET) has been suggested as a possible therapy for canine MF. To describe the use of TSET as palliative treatment for MF in a dog. An adult dog, previously diagnosed with nonepidermolytic ichthyosis, was presented with generalized erythroderma, alopecia and erosions. Histopathology revealed a densely cellular, well-demarcated, unencapsulated infiltrate extending from the epidermis to the mid-dermis compatible with MF. The infiltrate exhibited epitheliotropism multifocally for the epidermis, infundibula and adnexa. Due to a lack of response to chemotherapy, TSET was elected. Six megavoltage electrons were delivered using a 21EX Varian linear accelerator. A dose of 6 Gy was delivered to the skin surface and a 100 cm skin to surface distance was used for dog setup. The treatment time for the cranial half treatment was 3 h. The treatment was divided in two sessions (cranial and caudal halves of the body) 15 days apart. Clinical and histopathological complete remission was achieved and the dog was kept in remission with no additional treatments for 19 months before relapse and development of Sézary syndrome. To the best of the authors' knowledge, this is the first case reporting the use of TSET for medically refractory canine MF with post treatment follow-up. This case suggests that the use of TSET may be an effective palliative treatment for canine MF. © 2017 ESVD and ACVD.

    Objective: The aim of this in-vitro study was to evaluate the efficacy of bleaching products, determine the applicability and validation of the measurement methods. Materials and Methods: Freshly extracted 110 human incisor teeth were stained with whole blood and hemolysate solution prior to the application of 10 different home-bleaching products. Spectrophotometric measurements of the tooth shades were performed for each specimen before and after bleaching at the 1st, 3rd, 7th, and 14 days. Differences in lightness (Δl), chroma (Δc), hue (Δh) values and shade changes were measured to evaluate process. Computerized digital imaging analyses to determine the color changes were performed with Photoshop CS4 software (Adobe, San Jose, CA, USA). Statistical analyses were performed with analysis of variance, Scheffe and Tukey tests. Results: In all of the test groups regardless of the material used, a significant increase in lightness and hue, and decrease of chroma were observed, as compared to the control group. After recommended bleaching applications, Δl and Δh values respectively increased in group Zaris White and Brite (ZWB) and group Pola Night and Δc values showed significant decrease in groups ZWB and Rembrandt REM3 (P bleaching systems showed slower but almost permanent bleaching effect likewise office-based methods. Both software and spectrophotometric analyses have advantages such as evaluating the results objectively and numerically, also treatment outcomes could be preserved. PMID:25512738

    Full Text Available Mycosis fungoides (MF, the most indolent form of CTCL, originates from a clonal expansion of epidermotropic helper/memory T cells. Sezary syndrome (SS is a rare primay epidermotropic cutaneous T-cell lymphoma in leukemic. The aetiopathogenesis of MF and SS remains obscure despite several investigations. Infectious, environmental and genetic factors have been implicated as potential aetiological agents. The studies investigating the role of EBV in CTCL present conflicting results. The different sensitivities of the technical methods used in the evaluation of the presence of viral DNA or virus-related antigens make comparison of the results difficult. The aim of this study was to retrospectively evaluate the EBV-DNA load in skin biopsies from MF and SS patients by a highly sensitive (1-10 EBV-DNA copies/reaction quantitative-competitive PCR (QC-PCR developed in our lab to better asses the relationship between EBV and CTCL. Skin biopsies were obtained from 21 MF and 10 SS patients; skin biopsies from a 8 patients with inflammatory skin disease were used as controls. EBV-DNA was detected in 70% of biopsies from SS patients vs. 0% of MF patients. No control patients resulted EBV-DNA positive, as expected. In addition, in SS patients, the survival from diagnosis is lesser in EBV-positive patients vs.EBV-negative patients even if not statistically significant.We are going to investigate the presence of EBV-DNA in peripheral blood of a larger number of patients and to evaluate the pattern of viral genes expression, to better assess the aetiopathogenetical role of EB virus in this kind of neoplasies.

    Full Text Available Reading the popular culture may contribute to the reflexive view on a discipline such as archaeology. Film, as a part of popular culture, frequently unveils the hidden messages, which may be an echo of a discipline or its distorted image in the mirror. Film and archaeology share not only the common origins in the modernity, but also the imaginary spaces where the past and the present meet and intertwine. The subjects treated in films, the contexts in which archaeology appears, speak of the place the discipline holds in the society, reminding us at the same time of all the elements encompassed by the archaeological discourse. On the other hand, if we compare the portraits of the imaginary archaeologists (such as Professor Mihajlo Pavlović, Vera Zarić, with the witnesses of archaeology in Serbia over the 20th century (Nikola Vulić, Dragoslav Srejović, Milutin Garašanin, we shall approach the meeting point between academic and general public, science and the audience, theory and practice. Extraordinary individuals, unemployed dreamers living at the borders of the worlds, charming connoisseurs of the underworlds – these are but some of the qualities ascribed to the discipline by the films. However, these stereotypes do not generate out of the void, they are the consequence of the self-representation. This mystification of the discipline leads us back to the debate on the responsibility and ethics of the social scientists inside the society they live in. Of course, the suggested reading is one of the many possibilities, one of the archaeological interpretations.

    Purpose: Cutaneous T-cell lymphomas (CTCLs) are dominated by mycosis fungoides (MF) and Sézary syndrome (SS), and durable disease control is a therapeutic challenge. Standard total skin electron beam therapy (TSEBT) is an effective skin-directed therapy, but the possibility of retreatments is limited to 2 to 3 courses in a lifetime due to skin toxicity. This study aimed to determine the clinical effect of low-dose TSEBT in patients with MF and SS. Methods and Materials: In an open clinical study, 21 patients with MF/SS stages IB to IV were treated with low-dose TSEBT over <2.5 weeks, receiving a total dose of 10 Gy in 10 fractions. Data from 10 of these patients were published previously but were included in the current pooled data analysis. Outcome measures were response rate, duration of response, and toxicity. Results: The overall response rate was 95% with a complete cutaneous response or a very good partial response rate (<1% skin involvement with patches or plaques) documented in 57% of the patients. Median duration of overall cutaneous response was 174 days (5.8 months; range: 60-675 days). TSEBT-related acute adverse events (grade 1 or 2) were observed in 60% of patients. Conclusions: Low-dose (10-Gy) TSEBT offers a high overall response rate and is relatively safe. With this approach, reirradiation at times of relapse or progression is likely to be less toxic than standard dose TSEBT. It remains to be established whether adjuvant and combination treatments can prolong the beneficial effects of low-dose TSEBT.

    The treatment of advanced stage primary cutaneous T-cell lymphomas remains challenging. In particular, large-cell transformation of mycosis fungoides is associated with a median overall survival of two years for all stages taken together. Little is known regarding allogeneic hematopoietic stem cell transplantation in this context. We performed a multicenter retrospective analysis of 37 cases of advanced stage primary cutaneous T-cell lymphomas treated with allogeneic stem cell transplantation, including 20 (54%) transformed mycosis fungoides. Twenty-four patients (65%) had stage IV disease (for mycosis fungoides and Sézary syndrome) or disseminated nodal or visceral involvement (for non-epidermotropic primary cutaneous T-cell lymphomas). After a median follow up of 29 months, 19 patients experienced a relapse, leading to a 2-year cumulative incidence of relapse of 56% (95%CI: 0.38–0.74). Estimated 2-year overall survival was 57% (95%CI: 0.41–0.77) and progression-free survival 31% (95%CI: 0.19–0.53). Six of 19 patients with a post-transplant relapse achieved a subsequent complete remission after salvage therapy, with a median duration of 41 months. A weak residual tumor burden before transplantation was associated with increased progression-free survival (HR=0.3, 95%CI: 0.1–0.8; P=0.01). The use of antithymocyte globulin significantly reduced progression-free survival (HR=2.9, 95%CI: 1.3–6.2; P=0.01) but also transplant-related mortality (HR=10−7, 95%CI: 4.10−8–2.10−7; P<0.001) in univariate analysis. In multivariate analysis, the use of antithymocyte globulin was the only factor significantly associated with decreased progression-free survival (P=0.04). Allogeneic stem cell transplantation should be considered in advanced stage primary cutaneous T-cell lymphomas, including transformed mycosis fungoides. PMID:24213148

    Full Text Available Background: Primary cutaneous lymphomas (PCLs represent a heterogeneous group of T- and B-cell lymphomas that present in the skin with no evidence of extracutaneous disease at the time of diagnosis. The aim of this study was to assess and report the epidemiological characteristics of PCLs in Isfahan, Isfahan Province, Iran - as a main province of Iran. Materials and Methods: A total of 99 patients were recruited over a recent 10-year period (2003-2013 with diagnosis of PCLs; the patients were classified according to the The World Health Organization/European Organization for Research and Treatment of Cancer (WHO-EORTC criteria. Mean and standard deviations (SDs were used to describe continuous data, numbers, and percentages for categorical data. Statistical significance was defined as P < 0.05. Results: The patients comprised 45 men and 54 women aged 5-80 years (median 36 at diagnosis. The male-to-female ratio was 1:1.2. Histological examination showed features of primary cutaneous B-cell lymphomas (PCBCLs in four cases. The mean ± SD age in primary cutaneous T-cell lymphomas (PCTCLs and PCBCLs was 37.9 ± 16.5 years and 39.7 ± 9.1 years, respectively (P = 0.72. The mean ± SD latent period between the time of diagnosis and initiation of skin lesions in men and women was 2.3 ± 4.1 years and 5.9 ± 10.1 years, respectively (P = 0.02. The most frequent subtypes were mycosis fungoides (MFs (86.9% followed by Sιzary syndrome (SS (4%. Five patients died from PCL-related deaths. Conclusion: The distinguishing epidemiologic characteristics of PCL in Iran are the absence of a male predominance and a lower age of diagnosis. The study highlights the ethnic or regional variations in the clinicoepidemiological characteristics of PCLs.

    Serum or plasma specimens from 252 patients with lymphoid malignancies were screened for reactivity with adult T-cell leukemia virus-associated antigen (ATLA), and the relationship between the immunologic phenotype of the tumor cells and ATLA reactivity was determined. Anti-ATLA antibodies were found in 24 (29.3%) of 82 patients with T-cell malignancy. In contrast, the antibodies were found in none of the 106 patients with B-cell malignancy and only rarely in patients with other lymphoid malignancies without blood transfusions. Among the patients with T-cell malignancy, anti-ATLA antibodies were found in 23 (45.1%) of the 51 patients with OKT4-positive mature T-cell (inducer/helper T-cell) malignancy, but in none of the patients with T-cell malignancy of pre-T, thymic T-cell or OKT8-positive mature T-cell (suppressor/cytotoxic T-cell) phenotype. Furthermore, among the OKT4-positive mature T-cell malignancies, the antibodies were found in 16 (84.2%) of 19 patients with ATL and in 5 (27.8%) of 18 patients with mature (peripheral) T-cell lymphoma, in none of four with typical T-chronic lymphocytic leukemia, in one of nine with mycosis fungoides and in the one patient with small-cell variant of Sézary's syndrome. These results suggest that anti-ATLA positive T-cell malignancies with OKT4-positive mature T-cell phenotype must be the same disease, because it is highly possible that they have the same etiology and the same cellular origin. In the atypical cases, it seems necessary to demonstrate monoclonal integration of proviral DNA of ATLV or HTLV into the tumor cells in order to establish the final diagnosis of ATL.

    Full Text Available Pruritus and erythroderma could be a presenting sign of numerous internal malignancies. These symptomes can occur in the early stages of internal malignancies or precede them by months. On the other hand, these symptoms can also be caused by various other common conditions; therefore they are not specific for paraneoplastic diseases. This review overviews the current state of knowledge regarding paraneoplastic pruritus and paraneoplastic erythroderma, and highlights the association between these conditions and internal malignancies together with their epidemiological, clinical and diagnostic features. Paraneoplastic pruritus is often generalized with intractable and chronic course. Nocturnal pruritus can be a significant problem for these patients. Patients may present with secondary skin changes including excoriations, hyperpigmentation, lichenification and prurigo nodules. Paraneoplastic pruritus is more commonly known to be in association with Hodgkin lymphoma and other lymphomas, leukemia and polycythemia vera. Paraneoplastic erythroderma is more agressive and resistant to standart treatment modalities. Weakness, and significant weight loss are frequently seen as additional findings. It can be associated with fine scaling and hyperpigmentation (melanoerythroderma. Papuloerythroderma of Ofuji is distinct and rare clinical entity presenting with erythroderma. It is characterized by intensely pruritic and widespread red flat-topped papules. Cutaneous T-cell lymphoma, especially its leukemic variant Sézary syndrome are the most common cause of paraneoplastic erythroderma. A detailed and meticulous medical history, dermatologic and physical examination including an exam for lymph nodes should be the first step in the evaluation of paraneoplastic pruritus and erythroderma.     Full Text Available Artykuł prezentuje wykorzystanie rachunku kosztów działań do wyceny usług na trzech rynkach podlegających centralnym regulacjom – rynku usług pocztowych, telekomunikacyjnych oraz świadczeń zdrowotnych. Doświadczenia krajów Unii Europejskiej (w tym Polski pokazują nowe obszary wykorzystania rachunku kosztów działań. Potwierdzają, że zastosowanie tego narzędzia pozwala na kontrolę kosztów ponoszonych przy świadczeniu usług oraz wycenę na podstawie rzeczywistego rachunku ekonomicznego. Takie podejście gwarantuje przejrzystość procesu wyceny na rynkach, na których odgrywa to szczególnie istotną rolę. W części pierwszej przedstawiono zarys koncepcji rachunku kosztów działań wykorzystanej w PPUP Poczta Polska. Scharakteryzowano zastosowane rozwiązanie dostosowane do specyfiki działań poczty, zawierające elementy obiektowego rachunku kosztów działań oraz rachunku kosztów działań sterowanego czasem. W części drugiej zaprezentowano zastosowanie koncepcji rachunku kosztów działań do wyceny świadczonych usług w British Telecom. Wykorzystanie właściwych nośników kosztów pozwala na prawidłowe przypisanie kosztów obiektom je generującym. W części trzeciej opisano doświadczenia krajów Unii Europejskiej w zakresie stosowania rachunku kosztów działań do wyceny świadczeń medycznych. Stanowią one przesłankę dla wdrożenia tej koncepcji do wyceny usług zdrowotnych na rynku polskim.

  11. Outcome of Patients Treated With a Single-Fraction Dose of Palliative Radiation for Cutaneous T-Cell Lymphoma

    Purpose: Cutaneous T-cell lymphoma (CTCL) is a radiosensitive tumor. Presently, treatment with radiation is given in multiple fractions. The current literature lacks data that support single-fraction treatment for CTCL. This retrospective review assesses the clinical response in patients treated with a single fraction of radiation. Methods and Materials: This study reviewed the records of 58 patients with CTCL, primarily mycosis fungoides, treated with a single fraction of palliative radiation therapy (RT) between October 1991 and January 2011. Patient and tumor characteristics were reviewed. Response rates were compared using Fisher's exact test and multiple logistic regressions. Survival rates were determined using the Kaplan-Meier method. Cost-effectiveness analysis was performed to assess the cost of a single vs a multifractionated treatment regimen. Results: Two hundred seventy individual lesions were treated, with the majority (97%) treated with ≥700 cGy; mean follow-up was 41.3 months (range, 3-180 months). Response rate by lesion was assessed, with a complete response (CR) in 255 (94.4%) lesions, a partial response in 10 (3.7%) lesions, a partial response converted to a CR after a second treatment in 4 (1.5%) lesions, and no response in 1 (0.4%) lesion. The CR in lower extremity lesions was lower than in other sites (P=.0016). Lesions treated with photons had lower CR than those treated with electrons (P=.017). Patients with lesions exhibiting large cell transformation and tumor morphology had lower CR (P=.04 and P=.035, respectively). Immunophenotype did not impact response rate (P=.23). Overall survival was significantly lower for patients with Sézary syndrome (P=.0003) and erythroderma (P 200% higher than that for single-fraction radiation. Conclusions: A single fraction of 700 cGy-800 cGy provides excellent palliation for CTCL lesions and is cost effective and convenient for the patient

    zary Syndrome (SS/L-CTCL) is a rare but aggressive variant of cutaneous T cell lymphoma (CTCL), characterized by erythroderma, lymphadenopathy, and the presence of a circulating memory CD4(+) T cell malignant clone with a skin homing behavior, lacking CD26 and CD49d and over-expressing CD60. The availability of a panel of monoclonal antibodies recognizing distinct TCR-Vβ families, allows to typify the clone by flow cytometry in about 70 % of cases. The TCR-Vβ repertoire of 533 individuals, comprising 308 patients affected by CTCL, 50 healthy donors, and subjects affected by various non-neoplastic dermatological affections was evaluated by flow cytometry. Statistical analyses were performed using the SPSS statistical software package for Microsoft Windows (SPSS, version 21, Chicago, IL). TCR-Vβ2 levels below 5.4 % or above 39.5 %, within total CD4(+) T cells, showed the best balance between sensitivity (98.1 %) and specificity (96 %) to identify the presence of a clone in the peripheral blood of patients affected by SS. Based on this observation, a "two-step" procedure in the detection of the malignant T cell clone in CTCLs is herein suggested. TCR-Vβ2 assessment in all cases (first step). In the case of TCR-Vβ2 levels above 39.5 %, the presence of a clonal expansion of this family is suggested, deserving further confirmation by means of T cell gene rearrangement evaluation. In patients having a TCR-Vβ2 reactivity below 5.4 % (second step), the entire TCR-Vβ repertoire should be evaluated to typify the expanded clone. In conclusion, the single TCR-Vβ2 expression check, instead of the entire repertoire assessment, represents an easy and cost-effective method for the recognition of CTCL aggressive leukemic variant.

    Primary cutaneous lymphomas are uncommon. This article describes the Primary Cutaneous Lymphoma Registry of the Spanish Academy of Dermatology and Venereology (AEDV) and reports on the results from the first year. Disease registry for patients with primary cutaneous lymphoma. The participating hospitals prospectively recorded data on diagnosis, treatment, tests, and disease stage for all patients with primary cutaneous lymphoma. A descriptive analysis was performed. In December 2017, the registry contained data on 639 patients (60% male) from 16 university hospitals. The most common diagnoses, in order of frequency, were mycosis fungoides/Sézary syndrome (MF/SS) (348 cases, 55%), primary cutaneous B-cell lymphoma (CBCL) (184 cases, 29%), primary cutaneous CD30 + T-cell lymphoproliferative disorder (CD30 + CLPD) (70 cases, 11%), and other types of T-cell lymphoma (37 cases, 5%). In total, 105 (16.5%) of the cases recorded were incident cases. The most common diagnosis in the MF/SS group was classic MF (77.3%). Half of the patients with MF had stage IA disease when diagnosed, and the majority were either in partial remission (32.5%) or had stable disease (33.1%). The most widely used treatments were topical corticosteroids (90.8%) and phototherapy. The most common form of primary CBCL was marginal zone lymphoma (50%). Almost all of the patients had cutaneous involvement only and nearly half had stage T1a disease. Most (76.1%) were in complete remission. The main treatments were surgery (55.4%) and radiotherapy (41.9%). The most common diagnosis in patients with CD30 + CLPD was lymphomatoid papulosis (68.8%). Most of the patients (31.4%) had stage T3b disease and half were in complete remission. The most common treatments were topical corticosteroids (68.8%) and systemic chemotherapy (32.9%).     Full Text Available Abstract Background Recently, we clarified at the molecular level novel chromosomal translocation t(14;14(q11;q32 in a case of Sézary syndrome, which caused a rearrangement from TRAJ7 to the PPP2R5C gene. PPP2R5C is one of the regulatory B subunits of protein phosphatase 2A (PP2A. It plays a crucial role in cell proliferation, differentiation, and transformation. To characterize the expression and distribution of five different transcript variants of the PPP2R5C gene in leukemia, we analyzed the expression level of PPP2R5C in peripheral blood mononuclear cells from 77 patients with de novo leukemia, 26 patients with leukemia in complete remission (CR, and 20 healthy individuals by real-time PCR and identified the different variants of PPP2R5C by RT-PCR. Findings Significantly higher expression of PPP2R5C was found in AML, CML, T-ALL, and B-CLL groups in comparison with healthy controls. High expression of PPP2R5C was detected in the B-ALL group; however, no significant difference was found compared with the healthy group. The expression level of PPP2R5C in the CML-CR group decreased significantly compared with that in the de novo CML group and was not significantly different from the level in the healthy group. By using different primer pairs that covered different exons, five transcript variants of PPP2R5C could be identified. All variants could be detected in healthy samples as well as in all the leukemia samples, and similar frequencies and distributions of PPP2R5C were indicated. Conclusions Overexpression of PPP2R5C in T-cell malignancy as well as in myeloid leukemia cells might relate to its proliferation and differentiation. Investigation of the effect of target inhibition of this gene might be beneficial to further characterization of molecular mechanisms and targeted therapy in leukemia.

    Purpose: Cutaneous T-cell lymphoma (CTCL) is a radiosensitive tumor. Presently, treatment with radiation is given in multiple fractions. The current literature lacks data that support single-fraction treatment for CTCL. This retrospective review assesses the clinical response in patients treated with a single fraction of radiation. Methods and Materials: This study reviewed the records of 58 patients with CTCL, primarily mycosis fungoides, treated with a single fraction of palliative radiation therapy (RT) between October 1991 and January 2011. Patient and tumor characteristics were reviewed. Response rates were compared using Fisher's exact test and multiple logistic regressions. Survival rates were determined using the Kaplan-Meier method. Cost-effectiveness analysis was performed to assess the cost of a single vs a multifractionated treatment regimen. Results: Two hundred seventy individual lesions were treated, with the majority (97%) treated with ≥700 cGy; mean follow-up was 41.3 months (range, 3-180 months). Response rate by lesion was assessed, with a complete response (CR) in 255 (94.4%) lesions, a partial response in 10 (3.7%) lesions, a partial response converted to a CR after a second treatment in 4 (1.5%) lesions, and no response in 1 (0.4%) lesion. The CR in lower extremity lesions was lower than in other sites (P=.0016). Lesions treated with photons had lower CR than those treated with electrons (P=.017). Patients with lesions exhibiting large cell transformation and tumor morphology had lower CR (P=.04 and P=.035, respectively). Immunophenotype did not impact response rate (P=.23). Overall survival was significantly lower for patients with Sézary syndrome (P=.0003) and erythroderma (P<.0001). The cost of multifractionated radiation was >200% higher than that for single-fraction radiation. Conclusions: A single fraction of 700 cGy-800 cGy provides excellent palliation for CTCL lesions and is cost effective and convenient for the patient.

    Purpose: Recent trials with low-dose total skin electron beam (TSEB) therapy demonstrated encouraging results for treating primary cutaneous T-cell lymphoma (PCTCL). In this study, we assessed the feasibility of different radiation doses and estimated survival rates of different pathologic entities and stages. Methods and Materials: We retrospectively identified 45 patients with PCTCL undergoing TSEB therapy between 2000 and 2015. Clinical characteristics, treatment outcomes, and toxicity were assessed. Results: A total of 49 courses of TSEB therapy were administered to the 45 patients. There were 26 pathologically confirmed cases of mycosis fungoides (MF) lymphoma, 10 cases of Sézary syndrome (SS), and 9 non-MF/SS PCTCL patients. In the MF patients, the overall response rate (ORR) was 92% (50% complete remission [CR]), 70% ORR in SS patients (50% CR), and 89% ORR in non-MF/SS patients (78% CR). The ORR for MF/SS patients treated with conventional dose (30-36 Gy) regimens was 92% (63% CR) and 75% (25% CR) for low-dose (<30-Gy) regimens (P=.09). In MF patients, the overall survival (OS) was 77 months with conventional dose regimens versus 14 months with low-dose regimens (P=.553). In SS patients, the median OS was 48 versus 16 months (P=.219), respectively. Median event-free survival (EFS) for MF in conventional dose patients versus low-dose patients was 15 versus 8 months, respectively (P=.264) and 19 versus 3 months for SS patients (P=.457). Low-dose regimens had shorter treatment time (P=.009) and lower grade 2 adverse events (P=.043). A second TSEB course was administered in 4 MF patients with 100% ORR. There is a possible prognostic impact of supplemental/boost radiation (P<.001); adjuvant treatment (P<.001) and radiation tolerability (P=.021) were detected. Conclusions: TSEB therapy is an efficacious treatment modality in the treatment of several forms of cutaneous T-cell lymphoma. There is a nonsignificant trend to higher and longer clinical benefit

The two most common types of CTCLs are mycosis fungoides and the leukemic variant, Sézary syndrome. Prognosis of CTCLs depends on the stage, with a poor prognosis in advanced-stage disease. A number of agents have recently been developed for the treatment of CTCLs: chemotherapeutic agents such as pralatrexate, interferon-alpha, retinoids such as bexarotene, monoclonal antibodies such as alemtuzumab, and histone deacetylase inhibitors such as vorinostat and romidepsin. Nevertheless, there is no cure for CTCLs except for allogeneic stem cell transplant. A promising new drug is enzastaurin. Enzastaurin is a novel serine/threonine kinase inhibitor that binds to protein kinase C-β (PKC-β and inhibits the phosphoinositide-3 kinase (PI3K/AKT/phosphatase and tensin homolog (PTEN signaling pathway. Enzastaurin induces apoptosis and inhibits angiogenesis; it was also shown to suppress growth of CTCL cell lines in vitro. Given its low toxicity, enzastaurin has been tested against both solid tumors and hematologic malignancies. This article is focused on the potential role of enzastaurin in the treatment of CTCLs. A phase II multicenter trial evaluated enzastaurin monotherapy in patients with CTCLs. However, the results from this study were disappointing, demonstrating that enzastaurin had only modest clinical activity. Hence, enzastaurin is not currently developed for treating CTCLs. Predyspozycje obszaru do uprawiania różnych form turystyki potwierdza wynik oceny atrakcyjności wizualnej krajobrazu. W artykule scharakteryzowano zasoby krajobrazu kulturowego gminy Koronowo. Uzyskane wyniki wskazują, że niemal każda miejscowość posiada obiekt o dużej wartości historycznej bądź kulturowej oraz walory krajoznawcze. Znaczna ich część znajduje się w Koronowie, z uwagi na wielkość miasta i jego bogatą historię. Powszechna opinia dowodzi, że Zalew Koronowski stanowi największy walor turystyczny gminy Koronowo. Nie ulega wątpliwości, iż jest on najmocniejszym atutem oraz marką turystyczną. Świadczy o tym m.in. wzrost intensywności ruchu turystycznego w sezonie letnim. Na podstawie wyników analizy SWOT stwierdzono, że gmina Koronowo ma nadal nie w pełni wykorzystany potencjał turystyczny. Fakt ten ze względu na przeciętną wielkość presji turystycznej potwierdza potrzebę dalszych działań na rzecz rozwoju turystyki na analizowanym obszarze.   Abstract The gmina (third tier administrative unit of Koronowo is widely regarded an area of attractive landscape, as well as valuable and interesting tourist space. Its suitability for various forms of tourism is confirmed by the assessment of visual attractiveness of the landscape. The article describes the resources of the cultural landscape of Gmina Koronowo. The results indicate that almost every locality has an object of high cultural or historical value as well as sightseeing potential. Most of those objects, however, are located in Koronowo itself, given the size of the town and its rich history

We cannot miss this opportunity to point out that Faculty owe its existence, development and today high and internationally recognized rating to its founders from the industry, government, local community, and especially to the Faculty of Mechanical Engineering in Belgrade. Professors of the Faculty of Mechanical Engineering in Belgrade with their attitude towards science and education have implemented highly sensitive gyroscope into their ancestors which is moving them toward quality and creation of well accepted results in every social and R&D context. For development of thermal science, energy and energy efficiency science at MFKG twenty years ago, and for creation of conditions for education of experts in these areas, special tribute should be made to our dear professors: Academician Zoran Zarić, Ph.     Full Text Available Introduction The Garmab copper deposit is located northeast of Qaen (South Khorasan province in the1:100,000 scale map of Abiz in the eastern tectonic zone of Iran. It is hosted by Late Paleocene-Eocene lava flows consisting mainly of andesite, trachy¬andesite, andesite-basalt and basalt lavas, as well as pyroclastic rocks, including tuffs and ignimbrites. The Lut Block has undergone intense magmatic activitywith a variety of geochemical characteristics due to changing tectonic conditions (e.g., compression during subduction followed by tensional conditions; Karimpour et al., 2012; Zarrinkoub et al., 2012. The Lut Block has a great potential for the discovery of new mineral deposits, like the Mahrabad and Khonik porphyry copper-gold deposits (Malekzadeh shafarodi, 2009, the Dehsalam porphyry copper deposit (Arjmandzadeh, 2011, high sulfidation epithermal gold deposits such as Chah Shalghami (Karimpour, 2005 and IOCG deposits such as Kuh-E-Zar and Qaleh Zari (Mazlomi et al., 2008. Materials and methods After field studies of the Garmab area, 32 thin sections and 21 polished sections were prepared for petrological and mineralogical studies.In addition, 10 least-altered and fractured samples of volcanic rocks were selected for geochemical studies. Major oxides were determined using XRF analyses at the Zarazma laboratory. Induced polarization and resistivity geophysical data were collected and correlated with geological and alteration maps. The geophysical datawere collectedfrom 420 individual points, using a dipole-dipole arrangement along five profiles separated 60m apart.This covered the study area entirely. After a change in the mineralization trend was observed,additional profileswere designed, twoon bearings of 25º and three on 75º. Results The Garmab volcanic rocks exhibit typical geochemical characteristics of subduction zone magmas including strong enrichment in LILE and depletion in HFSE.
