    Lattice motivated triplet color scalar field theory is analyzed. We consider non-minimal as well as covariant derivative coupling with SU(2) gauge fields. Field configurations generated by external electric sources are presented. Moreover non-Abelian magnetic monopoles are found. Dependence on the spatial coordinates in the obtained solutions is identical as in the usual Abelian case. We show also that after a decomposition of the fields a modified Faddeev-Niemi action can be obtained. It contains explicit O(3) symmetry breaking term parameterized by the condensate of an isoscalar field. Due to that Goldstone bosons observed in the original Faddeev-Niemi model are removed. (orig.)

    Electrotherapy, including theTENS and interferential current (IFC) is one of the most frequently used treatments in physical therapy in patients with low back pain. The aim of this study was to assess the influence of TENS and IFC on pain relief and to compare the analgesic efficacy of the two currents. Sixty patients aged 53.5 ± 12.5, with low back pain, were randomly divided into two groups: IFC (gr. I) and TENS (gr. II). Depending on the groups, patients were given series of ten 20-minute sessions using either IF orTENS currents. In all patients VAS and Laitinen modified scale were taken before and after treatment. After 2-weeks therapy there was improvement in the VAS and Laitinen scale (all components) in both groups, except for the part of the Laitinen scale on the reduction of activity in the group II (TENS). There was no statistically significant difference between the TENS and IF groups in reducing the intensity and other aspects of pain (frequency, pain medication and activity limitation) under the influence of therapy (p > 0.05). Interferential current and TENS therapy are effective for pain relief in patients with low back pain. The study showed equal analgesic efficacy of both treatments.

    CHARM (CERN High energy AcceleRator Mixed field facility) is a completely new facility being built now in the east area building 157, and is currently under the commissioning phase. It is to be used as a facility for radiation testing electronics. CHARM uses a 24 GeV beam to hit a target creating a mixed field of radiation. The target is a rotating revolver with four different targets to choose from, two copper, one solid aluminum and one aluminum with holes in it. Four movable shields allows the user to change the field to simulate the radiation that would be received in LHC tunnel, adjacent areas, in space etc. A years worth of radiation exposure in the LHC could be simulated in only a few days in CHARM. There will be two conveyor systems for bringing electronics into the test position. One for small electronic boards mounted in a 300 × 300 × 200 mm box, and can have a maximum weight of 10 kg. This system is Called MONTRAC, and will automatically bring the test box on a shuttle from behind concrete shield...

    Multiple sclerosis is one of the most common demyelinating disease of the CNS connected with the autoimmune action. The effect of the disease is progressive disability, and one of the symptoms is pain. In relieving pain in the course of MS physical procedures and exercises of physiotherapy are used. The aim of the study was assessment of the pain in patients with the multiple sclerosis after applying laser radiation, magnetostimulation and kinesiotherapy. The studied material was consisted of 120 patients with multiple sclerosis of both sexes (82 women and 38 men) aged 21-81 years. Patients were randomly divided into 4 treatment groups and the assesment was performed three times. In the first group laser therapy, in the group II laser and magnetostimulation, in the third group kinesiotherapy, in the fourth group magnetostimulation was used. The same program of physiotherapy in all groups was used. All patients were performed the following tests to assess of the pain: The Laitinen Modified Questionnaire Indicators of Pain of and the Visual- Analogue Scale (VAS). In all treatment groups was observed tends to decrease a result of a point in The Laitinen Modified Questionnaire Indicators of Pain and the Visual-Analogue Scale (VAS). Correlation between groups demonstrated statistically significant result on the level ptreatment was applied towards group II assessed with parameter of the Questionnaire of Pain according to Laitinen, as well as towards group II and III assessed with parameter - of the Visual Analogue Scale (VAS). The good result, i.e. the reduction of the spot value, after the III examination towards the preliminary examination were got in the group II. Laser radiation is an effective method which has an analgesisc action. The combination of laser radiation and magnetostimulation reduces pain in patients with multiple sclerosis, and also allows to maintain a therapeutic effect even after the cessation of the application of these procedures, which indicates

    Two photon absorption laser induced fluorescence (TALIF) measurements of neutral hydrogen and its isotopes are typically calibrated by performing TALIF measurements on krypton with the same diagnostic system and using the known ratio of the absorption cross sections [K. Niemi et al., J. Phys. D 34, 2330 (2001)]. Here we present the measurements of a new calibration method based on a ground state xenon scheme for which the fluorescent emission wavelength is nearly identical to that of hydrogen, thereby eliminating chromatic effects in the collection optics and simplifying detector calibration. We determine that the ratio of the TALIF cross sections of xenon and hydrogen is 0.024 ± 0.001.

    In the present study, the heteroxenous life cycle of Sarcocystis species from three strains of the slaughtered sheep at Al-Azizia and Al-Saada abattoirs in Riyadh city, K.S.A., was studied. Muscle samples of the oesophagus, diaphragm, tongue, skeletal and heart muscles were examined. Varied natural infection rates in the muscles of the examined sheep strains were recorded as 83% in Niemy, 81.5% in Najdy and 90% in Sawakny sheep. Muscles of the diaphragm showed the highest infection level above all organs except Najdy sheep in which oesophagus has the highest rate. Also, the heart was the lowest infected organ (40% Niemy, 44% Najdy and 53% Sawakny). Microscopic sarcocysts of Sarcocystis arieticanis are easily identified in sections through the heart muscles of the domestic sheep Ovis aries (Artiodactyla: Bovidae). Cysts measured 38.5-64.4 μm (averaged 42.66 μm) in width and 62.4-173.6 μm (averaged 82.14 μm) in length. The validity of this species was confirmed by means of ultrastructural characteristics of the primary cyst wall (0.1-0.27 μm thick) which revealed the presence of irregularly shaped crowded and hairy-like projections underlined by a thin layer of ground substance. This layer consisted mainly of fine, dense homogenous granules enclosing the developing metrocytes and merozoites that usually contain nearly all the structures of the apical complex and fill the interior cavity of the cyst. Several septa derived from the ground substance divided the cyst into compartments. The merozoites were banana-shaped and measured 12-16 μm in length with centrally or posteriorly located nuclei. Experimental infection of carnivores by feeding heavily infected sheep muscles revealed that the dog, Canis familiaris, is the only final host of the present Sarcocystis species. Gamogony, sporogonic stages and characteristics of sporulated oocysts were also investigated.

    Full Text Available The Skyrme-Faddeev-Niemi (SFN model which is an O(3 σ model in three dimensional space up to fourth-order in the first derivative is regarded as a low-energy effective theory of SU(2 Yang-Mills theory. One can show from the Wilsonian renormalization group argument that the effective action of Yang-Mills theory recovers the SFN in the infrared region. However, the theory contains an additional fourth-order term which destabilizes the soliton solution. We apply the perturbative treatment to the second derivative term in order to exclude (or reduce the ill behavior of the original action and show that the SFN model with the second derivative term possesses soliton solutions.

    Low back pain syndromes most often occur due to overloading of the musculoskeletal system. The cause is a frequent, improper lifting of heavy objects, most commonly by those working physically, with repetitive movements of bending and straightening of the trunk (turning and bending with load). This problem affects not only adults but also children and adolescents. There is a growing interest in new forms of analgesic therapy nowadays, especially in those that exhibit synergistic therapeutic effects. The aim of this work is to evaluate the analgesic efficacy of magnetoledotherapy in patients with lumbar--sacrum spinal pain syndromes caused by joints degenerative changes. The examination was carried out in 66 patients of both sexes aged 30 to 76 (average 54.7 +/- 13.8) with low back pain syndrome caused by spinal degenerative changes. The patients were divided into three groups according to the applied analgesic therapy (magnetoledotherapy, magnetostimulation, TENS currents). Level of pain has been evaluated four times in all patients--before the start of therapy and after 5, 10 and 15 applications with the use of the modified Laitinen Questionnaire and Visual-Analoque Scale (VAS). Post therapy levels of pain in the studied patients decreased significantly. According to Laitinen questionnaire, the greatest improvement was observed in the group treated with magnetoledotherapy and TENS currents and the smallest improvement was observed in the group treated with magnetostimulation. 1. Magnetoledotherapy shows significant analgesic efficacy in patients with low back pain syndrome and shows no side effects. 2. Concurrent application of both the infrared radiation generated by LED's and magnetostimulation synergistically reinforces analgesic effect in patients with low back pain syndrome, especially in level of pain and frequency of its occurrence, which results in the increase of movement activity and decrease in administration of analgesics.

    As part of the work performed to model flow in the unsaturated zone at Yucca Mountain Nevada, a capillary hysteresis model has been developed. The computer program HYSTR has been developed to compute the hysteretic capillary pressure -- liquid saturation relationship through interpolation of tabulated data. The code can be easily incorporated into any numerical unsaturated flow simulator. A complete description of HYSTR, including a brief summary of the previous hysteresis literature, detailed description of the program, and instructions for its incorporation into a numerical simulator are given in the HYSTR user's manual (Niemi and Bodvarsson, 1991a). This report describes the incorporation of HYSTR into the numerical code TOUGH (Transport of Unsaturated Groundwater and Heat; Pruess, 1986).     The following paper covers evaluation of low-power laser therapy results in chronic Achilles tendon damage and external Epicondylalia (tennis elbow). Fifty patients with Achilles damage (18 women and 32 men, age average 30, 24 plus or minus 10, 39 years) and fifty patients having external Epicondyalgiae (31 women and 19 men, age average 44, 36 plus or minus 10, 88 years) have been examined. The patients were irradiated by semiconductor infrared laser wavelength 904 nm separately or together with helium-neon laser wavelength 632.8 nm. The results of therapy have been based on the patient's interviews and examinations of patients as well as on the Laitinen pain questionnaire. The results prove analgesic effects in usage of low- power laser radiation therapy can be obtained.

    Full Text Available This review essay takes a critical look at two recently published edited volumes, both focusing on the notion and problems of solidarity. Solidarity: Theory and Practice (Laitinen and Pessi, eds. attempts to unpack the complex idea of solidaristic practice by looking at a whole range of related concepts, such as the social brain, collective intentionality, empathy, work, and voluntary organizations. The Strains of Commitment: The Political Sources of Solidarity in Diverse Societies (Banting and Kymlicka, eds., on the other hand, focuses on a concrete problem: the generation and maintenance of redistributive solidarity within societies marked by diversity. Still, both volumes take a thorough and systematic look at existing scholarship on solidarity, and by encompassing both the theoretical and the empirical, mark a significant step forward in deepening our understanding of the role and place of solidarity in general social theory.

    The article is based on an interview with Juhani Hakkarainen of Vapo Oy. Wood pellets are used in Finland primarily to heat buildings such as schools and offices and in the home. They are equally suitable for use in larger installations such as district heating plants and power stations. According to him wood pellets are suitable for use in coal-fired units generating heat, power, and steam. Price-wise, wood pellets are a particularly competitive alternative for small coal-fired plants away from the coast. Price is not the only factor on their side, however. Wood pellets also offer a good environmental profile, as they burn cleanly and generate virtually no dust, an important plus in urban locations. The fact that pellets are a domestically produced fuel is an added benefit, as their price does not fluctuate in the same way that the prices of electricity, oil, coal, and natural gas do. The price of pellets is largely based on direct raw material and labour costs, which are much less subject to ups and downs

  12. Perfumed historic buildings: Issues of authenticity

    Full Text Available Contemporary historical preservation practice includes olfactory preservation as an experimental method of architectural preservation. The implementation of manufactured scents in historic buildings raises important issues of authenticity. This paper focuses on three important issues in the relation between olfactory preservation and authenticity: the importance of phenomenology in memory evocation; the relative character of the authenticity concept; and the significance of social values in historic preservation. This requires a critical examination of charters, documents and theoretical interpretations which reflect a broader concept of authenticity. The paper discusses certain articles of the Venice Charter, the Nara Document on Authenticity, as well as the sense of smell in architectural experience through critical analysis of the theories of John Ruskin, Viollet-le-Duc, Roger Scruton and Juhani Pallasmaa and their concepts of authenticity. Authenticity issues are illustrated by the examples of olfactory preservation: olfactory reconstruction of Philip Johnson’s Glass House; interior restoration and olfactory reconstruction of the Arts Club in Mayfair, London; and the creation process of the perfume brand Arquiste, a meaningful example which relocates the olfactory reconstruction context. These critical analyses raise the question of scent in historic buildings as a value in itself.

    Full Text Available Artykuł problematyzuje sposób, w jaki Giorgio Agamben rozprawia się z paradygmatem teologii politycznej w swoim cyklu Homo sacer. Autor porównuje koncepcje Agambena z tymi, które w ostatnich latach zaproponował Roberto Esposito – myśliciel, który wydaje się mieć wiele wspólnego z Agambenem. W rzeczywistości jednak, wybierając inną strategię intelektualną, idee Esposita mogą zostać użyte jako krytyczne narzędzia przeciwko niektórym aspektom projektu Agambena (np. przeciwko koncepcji profanacji. Wbrew deklaracjom Agambena oraz bezprecedensowego zasięgu i głębi jego studiów (a raczej: właśnie z powodu tychże, nie jest on w stanie zaproponować drogi wyjścia z reżimu teologii politycznej. Autor stara się udowodnić powyższą tezą, analizując terminologiczny poziom koncepcji Agambena, kierunek rozwoju jego myśli oraz sposób prowadzenia przez niego badań genealogicznych. W efekcie projekt Homo sacer zaczyna przypominać niemy język tego, co impolityczne.

    Full Text Available Objectives: Observational studies provide insights into real-life situations. Therefore, we assessed the effects of oral avocado/soybean unsaponifiable (ASU capsules on pain relief and functional ability in patients, while they were receiving a routine treatment for knee osteoarthritis (OA. Material and methods : An open, prospective, observational 6-month study was conducted in 99 centers in Poland in a group of 4822 patients with symptomatic knee OA receiving one 300 mg ASU capsule/day as a routine medication. The patients had no diagnoses of other rheumatic diseases and were not treated with other symptomatic slow-acting drugs for osteoarthritis (SYSADOAs. Data on OA symptoms and therapy were collected from the initiation of ASU treatment (visit 0 and during 3 consecutive control visits performed every 2 months (visits 1–3. Functional Lequesne index, severity of joint pain of one symptomatic knee (Laitinen index and VAS, use of analgesics and non-steroidal anti-inflammatory drugs (NSAIDs, adherence to treatment and adverse events were evaluated and recorded using electronic Case Report Forms. Results : Four thousand one hundred and eighty-six patients (86.8% attended all 4 visits. In 94.2% of patients (mean age 60.7 ±11.6 years SD, 73.4% female at least one OA risk factor was identified. There was a significant improvement in functional ability between the last and baseline visits as evidenced by the median Lequesne index decreasing from 8 to 4 points (p < 0.001. Measures of pain intensity also fell significantly (p < 0.001 throughout the study: median Laitinen score decreased from 6 to 3 points, median pain at rest VAS – from 1.8 to 0 cm and median pain during walking VAS – from 5.6 to 1.9 cm. The significant differences were also noted between consecutive visits. The proportion of patients using analgesics and NSAIDs declined from 58.8% at the baseline visit to 24.9% at the last visit 3 (p < 0.001. Defined daily dose of NSAIDs

  17. Suffering Daughters and Wives. Sentimental Themes in Finnish and Nordic Realism

    Full Text Available This article examines sentimental themes and scenarios in Nordic nineteenthcentury literature, focusing on Finnish realism. The main claim of the article is that the treatment of the Woman Question in Nordic literature is thematically connected to French sentimentalism that depicted upper-class women caught in the conflict between personal freedom and familial duties. Typical scenarios were family barrier to marriage and love triangle, in which an unhappily married woman fell in love with another man. French sentimental social novels took a stance on the position of women. Similar themes and scenarios can be found in Nordic nineteenth-century novels and plays. The ‘daughter novel’ tradition from Fredrika Bremer’s The President’s Daughters (1834 to Minna Canth’s Hanna (1886 depict the sufferings of upper-class girls in patriarchal family and society. A Doll’s House (1879 by Henrik Ibsen centers on the theme of conflicting duties, depicting the moral awakening of a doll-like wife, and Papin rouva (1893, ‘The Wife of a Clergyman’ by Juhani Aho concentrates on the sufferings and moral considerations of the unhappily married Elli. The article suggests that the sentimentalist legacy informs the Nordic nineteenth-century literature and should be taken into account in the scholarship.

    Back disorders are very common phenomena in modern society. One of the methods of spinal pain treatment is performing surgery. Unfortunately, this method is not one hundred percent effective. Some patients show no improvement after surgery, the pain persists and even increases. In these cases, it is reasonable to use the term Failed Back Surgery Syndrome (FBSS), i.e. back pain syndrome after unsuccessful spine surgery. The aim of the study is to assess the impact of rehabilitation on the pain intensity level in patients with FBSS. The study was conducted in a group of 38 patients aged from 32 to 87 (mean age 61 years), including 20 women and 18 men. All patients were operated for spinal pain syndrome. Afterwards, they underwent rehabilitation because of persisting pain after the surgery. For the pain assessment was used The Visual- Analogue Scale and The Laitinen Modified Questionnaire Indicators of Pain. The results were statistically analyzed. The study showed the high efficacy of specialized rehabilitation in patients with FBSS. The rehabilitation in patients with FBSS has a significant analgesic effect. Rehabilitation should be a gold standard in patients with FBSS.

  19. End-to-end probability for an interacting center vortex world line in Yang-Mills theory

    Full text: The understanding of quark confinement is a very important open problem in Yang-Mills theory. In this regard, nontrivial topological defects are expected to play a relevant role to achieve a solution. Here we are interested in how to deal with these structures, relying on the Cho-Faddeev-Niemi decomposition and the possibility it offers to describe defects in terms of a local color frame. In particular, the path integral for a single center vortex is a fundamental object to handle the ensemble integration. As is well-known, in three dimensions center vortices are string-like and the associated physics is closely related with that of polymers. Using recent techniques developed in the latter context, we present in this work a detailed derivation of the equation for the end-to-end probability for a center vortex world line, including the effects of interactions. Its solution can be associated with a Green function that depends on the position and orientation at the boundaries, where monopole-like instantons are placed. In the limit of semi flexible polymers, an expansion only keeping the lower angular momenta for the final orientation leads to a reduced Green function for a complex vortex field minimally coupled to the dual Yang-Mills fields. This constitutes a key ingredient to propose an effective model for correlated monopoles, center vortices and the dual fields. (author)

  1. An exploration of place: Towards an understanding of spatial character

    Full Text Available Architecture and its influences on our daily life form the premise of the study. Every space has a character which is defined by the type of human engagement it solicits. Spaces have varying characters and the suitability of their use depends on whether they are experienced as enchanting or disenchanting. Hermeneutic phenomenology, which emphasise the importance of context when the experience of a phenomenon is considered, guides the investigation. Since the 1970’s various architects have absorbed the hermeneutic phenomenology into their writings and work. Those referred to during the course of this paper include Christian Norberg-Schultz, Steven Holl, Peter Zumthor, Ziona Strelitz, Nabeel Hamdi and Juhani Pallasmaa. Christian Norberg-Schultz translated hermeneutic phenomenology into architecture in his theory defining the Sense of Place. This theory delineates that there are two spatial qualities influencing the experience of an environment, namely, ‘space’ and ‘character’. These qualities are interdependent and are therefore explored independently prior to their exploration as a singular whole.   An interdependent eco-system defining the relationship between the Context, User and Built Fabric is proposed within this paper. Observation and in-depth knowledge in all three areas is required for the creation of enchanted environments. If this system falls out of equilibrium, disenchanted environments develop. Enchanting environments engage their users and benefit the community and individuals alike. Disenchanted environments do not support their users, rendering their experience of architecture sombre and austere. The necessity of experiencing the world as a place, and not as a space, is a central theme throughout the paper.

    Full Text Available O estudo objetiva identificar temas que serão abordados no futuro em pesquisas de contabilidade gerencial, considerando-se a terceira metodologia de contabilidade gerencial proposta no estudo de Laitinen (2003, denominada de “Pesquisa em contabilidade gerencial com base no futuro”. Pesquisa descritiva foi realizada com abordagem quantitativa, por meio de questionário aplicado aos professores da área gerencial dos programas de pós-graduação em ciências contábeis do Brasil. Os resultados da pesquisa mostram que a função atual da contabilidade gerencial está focalizada no uso de recursos e na criação de valor. A tendência da contabilidade gerencial na opinião dos respondentes é a criação de valor. Prevêem que a contabilidade gerencial utilizará sistemas com dados monetários e não monetários, métodos de mensuração/avaliação e medidas de desempenho, filosofias e modelos de gestão. Como artefatos de contabilidade gerencial que serão mais utilizados apontaram: custeio variável, preço de transferência, retorno sobre o investimento e orçamento. Em relação à contabilidade financeira, acreditam em preponderância centrada na informação sobre produtos, projetos e planos de negócios.

  3. A Water Rehabilitation Program in Patients with Hip Osteoarthritis Before and After Total Hip Replacement.

    BACKGROUND Pain associated with coxarthrosis, typically occurring in middle-aged and elderly patients, very commonly causes considerable limitation of motor fitness and dependence on pharmacotherapy. This article provides an assessment of a rehabilitation program with tailored water exercises in patients with osteoarthritis before and after total hip replacement. MATERIAL AND METHODS A total of 192 patients (the mean age 61.03±10.89) suffering from hip osteoarthritis (OA) were evaluated before and after total hip replacement (THR). The clinical study covered measurements of hip active ranges of motion (HAROM) and the forces generated by pelvis stabilizer muscles. Pain intensity was assessed according to analogue-visual scale of pain (VAS) and according to the Modified Laitinen Questionnaire. The patients were divided into 6 groups (4 treatment and 2 control). We compared 2 rehabilitation programs using kinesitherapy and low-frequency magnetic field. One of them also had specially designed exercises in the water. Statistical analysis was carried out at the significance level α=0.05. This was a cross-sectional study. RESULTS A positive effect of water exercises on a number of parameters was found in patients with OA both before and after total hip replacement surgery. We noted a significant reduction of pain (pwater exercises most significantly reduced pain in patients with OA before and after total hip replacement surgery. 2. Inclusion of water exercises in a rehabilitation program can reduce the use of medicines in patient with OA and after THR.

    We propose the reformulations of the SU (N) Yang-Mills theory toward quark confinement and mass gap. In fact, we have given a new framework for reformulating the SU (N) Yang-Mills theory using new field variables. This includes the preceding works given by Cho, Faddeev and Niemi, as a special case called the maximal option in our reformulations. The advantage of our reformulations is that the original non-Abelian gauge field variables can be changed into the new field variables such that one of them called the restricted field gives the dominant contribution to quark confinement in the gauge-independent way. Our reformulations can be combined with the SU (N) extension of the Diakonov-Petrov version of the non-Abelian Stokes theorem for the Wilson loop operator to give a gauge-invariant definition for the magnetic monopole in the SU (N) Yang-Mills theory without the scalar field. In the so-called minimal option, especially, the restricted field is non-Abelian and involves the non-Abelian magnetic monopole with the stability group U (N− 1). This suggests the non-Abelian dual superconductivity picture for quark confinement. This should be compared with the maximal option: the restricted field is Abelian and involves only the Abelian magnetic monopoles with the stability group U(1){sup N−1}, just like the Abelian projection. We give some applications of this reformulation, e.g., the stability for the homogeneous chromomagnetic condensation of the Savvidy type, the large N treatment for deriving the dimensional transmutation and understanding the mass gap, and also the numerical simulations on a lattice which are given by Dr. Shibata in a subsequent talk.

The emphasis of this review is placed on how to obtain a manifestly gauge-independent picture for quark confinement supporting the dual superconductivity in the Yang-Mills theory, which should be compared with the Abelian projection proposed by 't Hooft. The basic tools are novel reformulations of the Yang-Mills theory based on change of variables extending the decomposition of the SU(N) Yang-Mills field due to Cho, Duan-Ge and Faddeev-Niemi, together with the combined use of extended versions of the Diakonov-Petrov version of the non-Abelian Stokes theorem for the SU(N) Wilson loop operator. Moreover, we give the lattice gauge theoretical versions of the reformulation of the Yang-Mills theory which enables us to perform the numerical simulations on the lattice. In fact, we present some numerical evidences for supporting the dual superconductivity for quark confinement. The numerical simulations include the derivation of the linear potential for static interquark potential, i.e., non-vanishing string tension, in which the "Abelian" dominance and magnetic monopole dominance are established, confirmation of the dual Meissner effect by measuring the chromoelectric flux tube between quark-antiquark pair, the induced magnetic-monopole current, and the type of dual superconductivity, etc. In addition, we give a direct connection between the topological configuration of the Yang-Mills field such as instantons/merons and the magnetic monopole. We show especially that magnetic monopoles in the Yang-Mills theory can be constructed in a manifestly gauge-invariant way starting from the gauge-invariant Wilson loop operator and thereby the contribution from the magnetic monopoles can be extracted from the Wilson loop in a gauge-invariant way through the non-Abelian Stokes theorem for the Wilson loop operator, which is a prerequisite for exhibiting magnetic monopole dominance for quark

    Context • Numerous modalities of therapeutic interventions exist for lumbar discopathy. Manual therapy is one option, although its effectiveness remains controversial. The addition of cryotherapy to manual therapy may enhance the health benefits in patients with lumbar discopathy.  Objective • The study intended to evaluate the efficacy of manual therapy combined with cryotherapy vs manual therapy combined with Kinesio taping for males with lumbar discopathy. Design • The research team designed a pilot randomized trial with concealed allocation, assessor blinding, and intention-to-treat analysis. Setting • The study occurred in the Physiotherapy Outpatient Department of the Regional Hospital (Zywiec, Poland). Participants • The participants were 40 males with lumbar discopathy, aged 30-75 y, who were patients in the department at the hospital. Intervention • The participants were randomly assigned to an intervention group that received Kaltenborn-Evjenth orthopedic manual therapy (KEOMT) combined with cryotherapy, the KEOMT-C group (n = 20), or to a control group that received KEOMT combined with Kinesio taping, the KEOMT-K group (n = 20). The participants in both groups received 10 treatments, 2 per wk for 5 wk. Outcome Measures • The primary outcome was measured using a visual analog scale and the Laitinen scale pain ratings. The secondary outcome measured the quality of life using the short form-36 questionnaire. The participants completed the tests at baseline and postintervention. Results • After the treatments, the intervention group had significantly lower scores than the control group for pain as well as significantly higher scores for quality of life. Conclusions • Patients achieved better health benefits from manual therapy when it was combined with cryotherapy.

  8. Validation of the Engineering Plant Analyzer methodology with Peach Bottom 2 stability tests

    The Engineering Plant Analyzer (EPA) had been developed in 1984 at Brookhaven National Laboratory to simulate plant transients in boiling water reactors (BWR). Recently, the EPA with its High-Speed Interactive Plant Analyzer code for BWRs ( ppercase HIPA-BWR ) simulated for the first time oscillatory transients with large, non-linear power and flow amplitudes; transients which are centered around the March 9, 1988 instability at the LaSalle-2 BWR power plant.The EPA's capability to simulate oscillatory transients has been demonstrated first by comparing simulation results with LaSalle-2 plant data (Wulff et al., NUREG/CR-5816, BNL-NUREG-52312, Brookhaven National Laboratory, 1992). This paper presents an EPA assessment on the basis of the Peach Bottom 2 instability tests (Carmichael and Niemi, EPRI NP-564, Electric Power Research Institute, Palo Alto, CA, 1978). This assessment of the EPA appears to constitute the first validation of a time-domain reactor systems code on the basis of frequency-domain criteria, namely power spectral density, gain and phase shift of the pressure-to-power transfer function.The reactor system pressure was disturbed in the Peach Bottom 2 power plant tests, and in their EPA simulation, by a pseudo-random, binary sequence signal. The data comparison revealed that the EPA predicted for Peach Bottom tests PT1, PT2, and PT4 the gain of the power-to-pressure transfer function with the biases and standard deviations of (-10±28)%, (-1±40)% and (+28±52)%, respectively. The respective frequencies at the peak gains were predicted with the errors of +6%, +3%, and -28%. The differences between the predicted and the measured phase shift increased with increasing frequency, but stayed within the margin of experimental uncertainty. ((orig.))

  10. Subjective evaluation of the effectiveness of whole-body cryotherapy in patients with osteoarthritis

    Full Text Available Objectives: One of the treatments for osteoarthritis (OA is whole-body cryotherapy (WBC. The aim of this study is to assess the effect of whole-body cryotherapy on the clinical status of patients with osteoarthritis (OA, according to their subjective feelings before and after the application of a 10-day cold treatment cycle. The aim is also to assess the reduction of intensity and frequency of pain, the reduction of the painkiller medication used, and to assess the possible impact on physical activity. Material and methods : The study involved 50 people, including 30 women (60% and 20 men (40%. Thirty-one patients had spondyloarthritis (62% of respondents, 10 had knee osteoarthritis (20%, and 9 hip osteoarthritis (18%. The overall average age was 50.1 ±10.9 years; the youngest patient was 29 years old and the oldest 73 years old. The average age of the women was 6 years higher. The study used a questionnaire completed by patients, and consisted of three basic parts. The modified Laitinen pain questionnaire contained questions concerning the intensity and frequency of pain, frequency of painkiller use and the degree of limited mobility. The visual analogue scale (VAS was used in order to subjectively evaluate the therapy after applying the ten-day treatment cycle. Results: According to the subjective assessment of respondents, after the whole-body cryotherapy treatments, a significant improvement occurred in 39 patients (78%, an improvement in 9 patients (18%, and no improvement was only declared by 2 patients (4%. Conclusions : Whole-body cryotherapy resulted in a reduction in the frequency and degree of pain perception in patients with osteoarthritis. WBC reduced the number of analgesic medications in these patients. It improved the range of physical activity and had a positive effect on the well-being of patients.

  11. [The use of Saunders lumbar traction in physiotherapy of patients with chronic lower back pain].

    Pain of the lower back is one of the most common ailments in modem society. Such frequent occurrence of back pain syndromes is a serious medical and social problem. Despite numerous attempts, there have not been comprehensive or decisive publications on the therapeutic standard for back pain syndromes that would cover all the aspects of the ailment. Partial report can change the attitude and shed new light on the treatment of these syndromes. The aim of this study was to evaluate the use of Saunders lumbar tractions in patients with chronic lower back pain in comparison with the group of patients undergoing pharmacological treatment. The study included 140 patients with chronic lower back pain who were divided into two groups. Each group consisted of 70 persons of mixed gender (the age mean value was 45). Group I were treated with the use of Saunders lumbar traction and Group II (control group) were treated pharmacologically. The following scales were used to assess pain and mobility of the lumbar part of the vertebral column (before treatment, in-treatment, right after treatment and 30 days after treatment): Laitinen, VAS, Rolland-Morris questionnaire and Schober functional test. The statistical analysis was performed with Greenhouse-Geisser test of within-subjects effects. In both the groups, a significant analgesic action and an improvement to spinal mobility were observed. Significantly better results were obtained in Group I where the patients had been treated with the use of Saunders lumbar traction. The pharmacological treatment applied in Group II showed an analgesic action and influenced positively the functional parameters of the patients. The effects, however, were much worse than in Group 1 where axial Saunders traction was applied to the lumbar part of the vertebral column.

  12. Estimating the effectiveness of pulmonary rehabilitation for COPD exacerbations: reduction of hospital inpatient days during the following year

The course took 6–8 weeks and included 11–16 supervised exercise sessions. Using electronic medical records, we studied all COPD patients with hospital admission in the city of Helsinki in 2014, including COPD diagnosis, criteria for exacerbation, and potential exclusion/inclusion criteria for PR.Results: Seventy-five of the patients finished the PR course and 92% of those patients showed clinically significant improvement. Their hospital days were reduced by 54% when compared to the year before. At 1 year after the course, 53% of the patients reported that they have continued with regular exercise training. In the city of Helsinki, 437 COPD patients were treated in a hospital due to exacerbation during 2014. On the basis of their electronic medical records, 57% of them would be suitable for PR. According to a rough estimate, 10%–20% hospital days could be saved annually if PR was available to all, assuming that the PR results would be as good as those shown here.Conclusions: The study showed that in a real-world setting, PR is efficient when measured by saved hospital days in severe COPD.   13. The effect of ergonomics in dentistry on the occurrence of pain in the cervical neck region of the spine

    Full Text Available Introduction: The problem of health risks resulting from the performance of the occupation of a dentist concerns, among other things, the cervical region of the spine. Due to the position assumed at work, this part of the spine is overloaded, and the degree of this overload depends, among other things, on the technique used by a dentist, the ergonomic conditions, patient adaptation skills, and cooperation with an assistant or assistants. Aim of the research : The objective of the study was to obtain an answer to the research questions posed: whether dentists possess knowledge concerning the principles of ergonomics; is there any relationship between the period of employment and cervical spine pain, age, technique of work, duration of performing work during the week, and pain in the neck region; and if dentists attach importance to the prophylaxis of musculoskeletal disorders. Material and methods : The study covered 52 dentists – 33 females (63.5% and 19 males (36.5%, and was conducted using an anonymous questionnaire designed by the authors, the Visual Analogue Scale (VAS, a modified pain assessment questionnaire according to Laitinen, and methods of descriptive statistics (Pearson’s χ 2 test for independence. Results : Physicians possess knowledge concerning the ergonomics of work. In addition, more than 60% of respondents possess modern, electrically adjustable equipment, and the technique of work depends on the age of the dentist. There is a relationship between cervical spine pain and the duration of performance of the occupation (p = 0.01122. According to dentists (48.1%, pain in the neck region of the spine is caused by long-lasting maintenance of a static position of the body, and kinesitherapeutic exercises alleviate these complaints in 23.1% of respondents. Conclusions : Despite the use the principles of ergonomics at work, dentists are exposed to the occurrence of pain in the neck region of the spine.

    Multiple sclerosis is a chronic, inflammatory, demyelinating disease of the central nervous system, which is characterized by diverse symptomatology. Most often affects people at a young age gradually leading to their disability. Looking for new therapies to alleviate neurological deficits caused by the disease. One of the alternative methods of therapy is high - tone power therapy. The article is a comparison of high-tone power therapy and kinesis in improving patients with multiple sclerosis. The aim of this study was to evaluate the effectiveness of high-tone power therapy and exercises in kinesis on the functional status of patients with multiple sclerosis. The study involved 20 patients with multiple sclerosis, both sexes, treated at the Department of Rehabilitation and Physical Medicine in Lodz. Patients were randomly divided into two groups studied. In group high-tone power therapy applied for 60 minutes, while in group II were used exercises for kinesis. Treatment time for both groups of patients was 15 days. To assess the functional status scale was used: Expanded Disability Status Scale of Kurtzke (EDSS), as well as by Barthel ADL Index. Assessment of quality of life were made using MSQOL Questionnaire-54. For the evaluation of gait and balance using Tinetti scale, and pain VAS rated, and Laitinen. Changes in muscle tone was assessed on the basis of the Ashworth scale. Both group I and II improved on scales conducted before and after therapy. In group I, in which the applied high-tone power therapy, reported statistically significant results in 9 out of 10 tested parameters, while in group II, which was used in the exercises in kinesis an improvement in 6 out of 10 tested parameters. Correlating the results of both the test groups in relation to each other did not show statistically significant differences.     The Romanian Association of Medical Laboratories (RAML) conferences have acquired a reputation for standing out as the most prominent and efficient meetings in the national community of laboratory medicine, being a landmark of the development in this field in Romania and an active affiliation to international forums. This year, the conference setting was Piatra Neamt, in the northeast part of Romania, which produced a friendly and stimulating professional environment. As in previous years, leading experts in the fields of laboratory medicine attended the event. This year, we enjoyed the opportunity to have such distinguished guests as the members of the executive board of International Federation of Clinical Chemistry and Laboratory Medicine (IFCC); Graham Beastall, IFCC President; Päivi Hannele Laitinen, IFCC secretary; and Grazyna Sypniewska, IFCC Communication and Publication Division, and editor of the electronic journal of the IFCC. As usual, the conference program included all aspects of clinical laboratory activity, with a special focus on technology development, instrumentation and laboratory management. Fully aware of the fact that the complexity and depth of laboratory practice have undergone an impressive and rapid evolution, the specific goals of the event were to increase knowledge in the fundamentals of new molecular investigation, areas which show the tendency to become routine in our daily activity. In addition, laboratory management and the place of medical laboratories in the process of translational medicine were subjects of focus. The 6th Conference of the Romanian Association of Medical Laboratories was held from Wednesday 1st to Saturday 4th of June 2011. A total of 273 participants from all local branches of the Association attended. The scientific program included seven plenary sessions where 22 lectures and 18 short communications were delivered, and three poster sessions with 44 poster presentations.   17. High Adherence to CPAP Treatment Does Not Prevent the Continuation of Weight Gain among Severely Obese OSAS Patients

    Study Objectives: Obstructive sleep apnea syndrome (OSAS) patients benefit from continuous positive airway pressure (CPAP) treatment in a dose-response manner. We determined adherence and weight control, as well as their predictors, among long-term CPAP users. Methods: Cohort of 1,023 OSAS patients had used CPAP on average of 6.6 ± 1.2 years. BMI was determined at baseline and at follow-up visits. There were 7.4 ± 1.7 BMI and 6.5 ± 1.8 CPAP usage measurements per patient on average. Using the Bayesian hierarchical model, we determined the patients' individual trends of BMI and adherence development. Patients with significantly increasing or decreasing trends were identified at the posterior probability level of > 90%. Results: The mean age in the cohort was 55.6 ± 9.8 years, BMI 33.5 ± 6.4 kg/m2, apnea-hypopnea index 33.7 ± 23.1, and CPAP usage 6.0 ± 1.8 h/day. The majority of patients had no significant change in BMI (mean annual weight gain 0.04 ± 0.29 kg/m2) or CPAP adherence (mean annual increase 11.4 ± 7.0 min/day). However, at the individual level, 10% of the patients showed significant annual weight gain (0.63 ± 0.35 kg/m2) during the 5-year follow-up period. At baseline these patients were already more severely obese (mean BMI 40.0 ± 5.9 kg/m2) despite being younger (mean 50.9 ± 9.5 years) than the rest of the cohort. Conclusions: In the majority of CPAP-treated OSAS patients, weight did not significantly change but gained slightly slower than in age-matched population in general. However, in 10% of patients, high adherence to CPAP treatment did not prevent the continuation of weight gain. These patients present a high-risk group for OSAS-related multimorbidity later in life.     Troublesome heel spur is a nuisance condition that affects people of all ages. Treatment of patients with heel spur is a difficult and lengthy process requiring patience from both the patient and the therapist. Sometimes, the only and ultimate method of treatment is surgery, although spurs tend to recur. The aim of the study is a comparative analysis of the analgesic efficacy of ultrasound and shock wave therapy in patients with heel spur. The cause of pain in the course of calcaneal spur is inflammation of the attachment of the plantar fascia, which plays an important role in the process of walking and is seriously strained during different types of movement. Treatment of patients is a difficult and lengthy process. The study was conducted on a group of 47 patients of both sexes, aged 38-60 years (mean 51.3) with a plantar calcaneal spur confirmed by X-ray images. Patients were randomly assigned into two groups using a simple randomization: Group 1-ultrasound therapy group (a series of ten treatments) and Group 2-the radial shock wave group (series of four treatments). In all patients, pain intensity was assessed three times: before therapy, after the first and second weeks of treatment. A version of Laitinen's pain assessment questionnaire and the Huskisson visual analogue scale (VAS) were used. Of the group of studied respondents, 47 patients of both sexes and aged 38-60 years (mean age 51.3) with a heel spur (confirmed on X-rays), who had pain for at least a month, were randomly included in the study. The patients were classified into: Group 1-US therapeutic group (a series of ten treatments) and Group 2-with RSWT (a series of five treatments). Pain intensity was assessed three times: before the treatment, after the first and second week of the treatment with the application of the VAS and the Leitinen Pain Questionnaire.     Increased expression of degenerative disease of the lumbar spine is an onerous task, which reduces the efficiency of the activity and life of many populations. It is the most common cause of medical visits. In 95% of cases the cause of complaints is a destructive process in the course of degenerative intervertebral disc called a lumbar disc herniation. Protrusion of the nucleus pulposus causes severe pain and impaired muscle tone, often more chronic and difficult to master. Successful treatment of lumbar disc herniation constitutes a serious interdisciplinary problem. It is important to properly planned and carried out physiotherapy. Based on the number of non-invasive methods, to reduce muscle tension, mute pain and alleviation of inflammation. It is the treatment safe, effective, and at the same time, which is their big advantage, readily available and cheap. It is worth noting that not every method has the same efficiency. The question that the methods are effective in relieving pain and helping to effectively increase the range of motion led to a comparison of two methods - Low Level Laser Therapy (LLLT) and pulsating magnetic field therapy. The aim of the study was to compare the efficacy of LLLT and pulsating magnetic field therapy in combating pain and increase range of motion of the spine of people with degenerative spine disease of the lower back. 120 patients with diagnose lumbar disc herniation whit no nerve roots symptoms. Patients were divided into two Groups: A and B. Group A of 60 patients were subjected to laser therapy (λ=820nm, P=400mW, Ed=6-12 J/cm²) and the second Group B of 60 patients too, to pulsating magnetic fields procedures (5mT, 30 Hz, 15 minutes).     Full Text Available In the 1990s, Juhani Pallasmaa wrote a compact, yet, eye-opening book expressing his growing concern about the architectural profession’s waning ability to reference all the senses of the body in the design process. Bit by bit, the perceptive chasm between the architect and architecture was widening both in the design process and in the cognitive experience of existing architecture. More than a decade later, these concerns have heightened as the ramifications of this design divide appear in the form of architecture whose divorced virtual quality has spilled over from the design process into built reality. The illusion of virtual reality – as achieved by 3D simulation in all its glories – has pulled the architect into a zone of false confidence where he/she feels that the design has come to life before it is built and that every corner and detail can be simulated and therefore understood. But can it be touched, smelt, tasted or heard? In fact, is even what we see on the screen anywhere close to what we see as we move bodily through its spaces? This phenomenon is addressed in a design studio run by the two authors of this paper. The purpose of the design studio, which is held in the Department of Architectural Engineering, of the Faculty of Engineering of the British University in Egypt, is to design a community centre linked to a place of worship within a residential compound currently under construction on the outskirts of Cairo. The graduating class of ten students is in the final and fourth year of a program that emphasized the engineering and project management aspects of architecture at the expense of theory and history of art and architecture. The university has no humanities or liberal arts program as yet, and students have minimal contact to arts within the university system. Aim of the study The aim of this study was to evaluate the effectiveness of TENS in the treatment of low back pain. Material and Methods Study included of forty patients aged 30 - 60 years (mean age 45 years. Immediately before and after therapy in all patients: assessment of pain using the VAS assessment of pain intensity and frequency, amount of applied analgesics, limiting physical activity using a questionnaire Laitinen, Schober test assessing the scope bending lumbosacral spine. Statistical analysis was performed using version 1.4.8 PQStat package. In order to compare the results obtained in the test prior to treatment with the results obtained after treatment  was Wilcoxon test at the significance level α = 0.05. Results Achieved a statistically significant reduction in the intensity and frequency of pain, reduction of applied analgesics, reduced physical activity limitations and increasing the range of flexion of low back pain. Conclusion TENS  is an effective treatment for both acute and chronic low back pain   Key words: transcutaneous electrical nerve stimulation, low back pain.

    Full Text Available In this paper, family social work is constructed through the analysis of social service discourses from the social workers’ perspective. Recent research shows how social workers are dealing with complex and fluid issues, as well as the societal uncertainty in their work with families (e.g., Spratt, 2009; Menéndez et al., 2015. Based on earlier studies, it is vital to analyse family social work in different contextual settings. Societal, political and organisational contexts affect the preconditions of social work, but social work also needs to operate within structures (e.g., Pohjola et al., 2014. This paper provides insights into the Lithuanian family social work. The focus is on what kinds of features construct Socialinis darbas su šeima Roberta Motiečienė, Merja Laitinen 12 family social work by analysing social workers’ discourses. This analysis continues the research of Eidukevičiūtė (2013, who analysed family social work practices in transitional Lithuanian society. This researcher aimed to deepen the knowledge about child protection services in Lithuania, the father’s role in child care and the mother’s performance in it. According to Eidukevičiūtė (2013, social workers are still struggling in the field of family social work. This study continues the research tradition in the field of family social work, paying attention to the different contextual settings where family social work is conducted. The Lithuanian government has stated that family policy is a key component of its mandate where (Social Report, 2014. The Council of Social Work plays a very important role in providing guidance on how to implement the government’s policy in the field of family social work. The European Commission Council (2015 provides recommendations for the implementation of the 2015 National Reform Programme, which should concentrate on the people (30% of the total population who are at risk of poverty.
