    Introduction Les tumeurs des glandes salivaires sont des tumeurs rares représentant 3à 5% des tumeurs de la tête et du cou. La classification de l'OMS 2005 distingue les tumeurs épithéliales, les tumeurs mésenchymateuses, les tumeurs hématologiques et les tumeurs secondaires. Méthodes Notre travail consiste en une étude rétrospective réalisée sur une période de 10 ans allant de janvier 2002 à janvier 2012. Les critères d'inclusion étaient: l'âge, le sexe, le siège de la tumeur et le type histologique. Résultats L'incidence annuelle des tumeurs malignes primitives des glandes salivaires dans notre série était de 15 cas par an. Cent cinquante quatre cas de tumeurs malignes primitives des glandes salivaires ont été colligés sans prédominance de sexe (78 femmes (50,6%) et 76 hommes (49,4%)). La moyenne d'âge était de 60 ans avec des extrêmes de 4 et 83 ans et un pic de fréquence entre 51et 70 ans. Deux tiers des cas (65%) avaient une localisation au niveau des glandes principales avec 66 cas au niveau de la parotide (43%) et 34 cas au niveau de la glande sous maxillaire (22%). Cinquante quatre patients avaient une tumeur maligne des glandes salivaires accessoires (35%) dont 61% au niveau du palais. Aucun cas de tumeur maligne de la glande sublinguale n'a été recensé dans notre étude. Le type histologique prédominant dans notre série était le carcinome adénoïde kystique et retrouvé chez 43 patients (27,9%), suivi de l'adénocarcinome sans autre indication chez 37 patients (24%) puis du carcinome mucoépidermoïde chez 16 patients (10,4%) et de l'adénocarcinome polymorphe de bas grade également chez 16 patients (10. 4%). Conclusion Les tumeurs malignes des glandes salivaires représentent un ensemble hétérogène de maladies de caractérisation complexe et de fréquence variable. PMID:25120861

    Le lymphome de Burkitt (LB) est une forme de lymphome malin non-Hodgkinien qui provient de l'évolution maligne et de la prolifération de cellules lymphoïdes de type B. Le diagnostic positif repose sur la biopsie d'une masse tumorale ou la ponction de la moelle osseuse révélant la présence de cellules tumorales. Le cas rapporté est celui d'un jeune homme d'une vingtaine d'années qui a été adressé pour des tuméfactions gingivales évoluant depuis 1 mois suite à des extractions dentaires. L'examen anatomopathologique après biopsie complété par l'immunohistochimie conclut à un lymphome de Burkitt. La prise en charge a consisté en une chimiothérapie. Bien que rare, le lymphome de Burkitt est une tumeur agressive qui représente un véritable problème de santé publique d'où la place importante qu'occupe le médecin dentiste dans le diagnostic précoce afin de permettre une prise en charge rapide et appropriée seule garante de la guérison.     Les douleurs lombaires relèvent d'étiologies multiples dont le diagnostic peut être source de grandes difficultés. Le lymphome rachidien primitif est rare et son diagnostic nécessite une biopsie souvent scanoguidée. Un homme de 30 ans, était hospitalisé pour lombalgies inflammatoires évoluant dans un contexte d'altération de l'état général avec à l'examen des douleurs à la palpation des apophyses épineux L2L3, sans syndrome tumoral périphérique. Le bilan biologique montrait un syndrome inflammatoire. Le bilan morphologique était en faveur d'une spondylodiscite. La première biopsie montrait une ostéite granulomateuse. L'aggravation clinique et radiologique sous anti bacillaire a mené à reconsidérer le diagnostic et la deuxième biopsie confirme le diagnostic du lymphome. Le diagnostic de tuberculose osseuse en particulier vertébrale nécessite une confirmation bactériologique et ou histologique pour ne pas méconnaître un lymphome osseux primitif. PMID:28292061

    Management of diagnosing malignant lymphomas has changed with development of CT-guided techniques and reliable biopsy tools.Pathologists can use representative tissue samples for sub classification in more than 90%.Evaluation of residual lymphoma or relapse can be nearly as effective. Therefore percutaneous biopsy can be considered as primary diagnostic tool in the absence of peripheral lymphadenopathy. CT-guided biopsies can be performed on an outpatient basis under conscious sedation considering contraindications as well as regional complications. Acceptance of percutaneous biopsy by the pathologist and oncologist is based on diagnostic effectiveness that is significantly improved if more than 3{dagger}solid tissue samples are taken. This article reviews the value of CT-guided biopsy in comparison to surgical procedures in patients with malignant lymphoma. Essential aspects that lead to a diagnostic percutaneous biopsy are discussed on grounds of the current literature. (orig.) [German] Mit der Entwicklung CT-gesteuerter Biopsieverfahren sowie zuverlaessiger Biopsiesysteme hat sich die Vorgehensweise bei der Diagnostik maligner Lymphome geaendert. Repraesentative Stanzzylinder mit hoher Praeparatequalitaet ermoeglichen dem Pathologen eine Subklassifikation der Lymphome in ueber 90%. Eine aehnlich hohe Treffsicherheit ist auch bei der Rezidivdiagnostik moeglich. Deshalb sind perkutane Biopsien bei fehlender peripherer Lymphadenopathie als primaere diagnostische Methode geeignet. Unter Beachtung der Kontraindikationen sowie der regional unterschiedlichen Komplikationsmoeglichkeiten koennen die meisten CT-gesteuerten Biopsien ambulant unter Analgosedierung durchgefuehrt werden. Die Akzeptanz perkutaner Biopsien durch Pathologen und Onkologen ist an die diagnostische Genauigkeit gekoppelt.     Primär kutane B-Zell-Lymphome (PCBCL) beschreiben reifzellige lymphoproliferative Erkrankungen der B-Zell-Reihe, die primär die Haut betreffen. Die Biologie und der klinische Verlauf der einzelnen PCBCL-Subtypen variieren untereinander stark und unterscheiden sich grundsätzlich von primär nodalen und systemischen B-Zell-Lymphomen. Primär kutane Marginalzonenlymphome (PCMZL) und primäre kutane follikuläre Keimzentrumslymphome (PCFCL) werden auf Grund ihres unkomplizierten Verlaufs und ihrer exzellenten Prognose zu den indolenten PCBCL gezählt. Demgegenüber stellen die diffus großzelligen B-Zell-Lymphome, hauptsächlich vom Beintyp (DLBCL, LT) die aggressiveren PCBCL-Varianten mit schlechterer Prognose dar. Für die Ausbreitungsdiagnostik und die Therapieentscheidung sind eine genaue histologische und immunhistochemische Klassifizierung sowie der Ausschluss einer systemischen Beteiligung in Abgrenzung zu nodalen oder systemischen Lymphomen notwendig. Die Diagnostik sollte dabei durch molekularbiologische Untersuchungen unterstützt werden. Therapeutisch stehen für die indolenten PCBCL primär operative und radioonkologische Maßnahmen im Vordergrund sowie eine Systemtherapie mit dem CD20-Antikörper Rituximab bei disseminiertem Befall. Die aggressiveren Varianten sollten in erster Linie mit Kombinationen aus Rituximab und Polychemotherapieschemata wie z. B. dem CHOP-Schema oder Modifikationen davon behandelt werden. Auf Grund der in allen seinen Einzelheiten noch nicht vollständig verstandenen Pathogenese und Biologie sowie des begrenzten Therapiespektrums der PCBCL besteht hier, speziell beim DLBCL, LT, noch erheblicher Forschungsbedarf. © 2016 Deutsche Dermatologische Gesellschaft (DDG).   13. Intraläsionale Therapie niedrig maligner primär kutaner B-Zell-Lymphome mit Anti-CD20-Antikörper: Nebenwirkungen korrelieren mit gutem klinischen Ansprechen.

    Die intraläsionale Gabe von Anti-CD20-Antikörpern (Rituximab) wurde als effektive Therapieoption für Patienten mit niedrig malignen primär kutanen B-Zell-Lymphomen beschrieben. Bis heute wurden allerdings keine Parameter identifiziert, welche reproduzierbar ein gutes klinisches Ansprechen dieser Therapie vorhersagen. Ziel dieser Studie ist, sowohl das klinische Ansprechen und die unerwünschten Nebenwirkungen als auch die Patientenwahrnehmung hinsichtlich intraläsionaler Injektionen von anti-CD20-Antikörpern zur Behandlung indolenter primär kutaner B-Zell-Lymphome im Vergleich mit anderen Therapien zu evaluieren. Elf Patienten mit einem primär kutanen B-Zell-Lymphom, namentlich primär kutanes Keimzentrumslymphom (n = 9) und primär kutanes Marginalzonenlymphom (n = 2), welche mittels intraläsionalem Anti-CD20-Antikörper behandelt wurden, wurden retrospektiv evaluiert hinsichtlich der Ansprechrate und unerwünschter Nebenwirkungen sowie in Bezug auf deren Selbsteinschätzung dieser und anderer Therapien des primär kutanen B-Zell-Lymphoms. Patienten, deren primär kutanes B-Zell-Lymphom mittels intraläsionaler Gabe von Anti-CD20-Antikörper behandelt wurde, zeigten ein komplettes oder partielles Ansprechen in 45 % beziehungsweise 27 % aller Patienten. Speziell Patienten mit grippeähnlichen Symptomen nach erfolgter Injektion zeigten ein gutes Ansprechen. Die Mehrheit der Patienten empfand die Therapie mit Rituximab als die beste Therapie im Vergleich zu anderen Therapien wie beispielsweise chirurgische Exzision oder Radiotherapie. Intraläsionales Rituximab ist eine effektive Therapie mit hoher Patientenzufriedenheit. Starke therapiebedingte Nebenwirkungen wie Fieber, Schüttelfrost und Kopfschmerzen nach Gabe von Rituximab könnten als Indikator für gute Wirksamkeit dienen. © 2017 Deutsche Dermatologische Gesellschaft (DDG).     Résumé Le sarcome à cellules claires du rein (SCCR) se voit très rarement chez les jeunes. Il est caractérisé par une évolution agressive marquée par un taux élevé de récidive et de mortalité. Nous rapportons le cas d’un SCCR chez un patient de 17 ans et nous discutons de son apport et de son intérêt médical en vue d’une bonne prise en charge thérapeutique. L’agressivité du SCCR et la prolifération de métastases surtout osseuses impliquent qu’il ne faut pas méconnaître ce diagnostic afin de mettre en place un traitement adapté. PMID:24940474

    Chemotherapy represents the most important therapeutic option in malignant lymphomas. Low to intermediate risk Hodgkin's disease is treated by a combination of chemotherapy and radiation. The new chemotherapy protocol BEACOPP has improved the outcome of advanced stages in comparison with the internationally accepted standard protocol COPP/ABVD. Dependent on the initial staging, cure rates between 50 and 95% can be achieved. Indolent non-Hodgkin's lymphomas usually present in advanced stages of disease. Chemotherapy in these cases has palliative character and aims at improving patients'quality of life and at avoiding complications due to the disease. In aggressive and very aggressive non-Hodgkin's lymphoma chemotherapy is curative and must be initiated immediately irrespective of the staging results. The efficacy of the standard protocol CHOP (cyclophosphamide,doxorubicin, vincristine and prednisone), that was established in the 1970s, has recently been improved by shortening of the therapy interval (CHOP-14 vs.CHOP-21),addition of etoposide (CHOEP) and combination with the monoclonal antibody rituximab (R-CHOP). The value of high dose chemotherapy with stem cell transplantation has been shown unequivocally only for aggressive non-Hodgkin lymphoma and relapsed Hodgkin's disease responsive to chemotherapy. The therapeutic strategy of malignant lymphomas is likely to be improved within the next years due to the introduction of novel cytostatic agents, the broadening application of monoclonal antibodies,upcoming new transplantation procedures and the development of substances with molecular targets.To rapidly increase our current knowledge on the topic it is mandatory to include patients into the large national and international multicenter studies.     L'atteinte cardiaque au cours des lymphomes lymphoblastiques est rare. Il s'agissait d'un patient de 17 ans de race noire qui a été admis pour une douleur médiothoracique. Le patient avait des signes d'insuffisance cardiaque droite; un syndrome d’épanchement pleural liquidien gauche; des adénopathies superficielles et une splénomégalie de type IV de Hackett. La biopsie ganglionnaire a été réalisée pour la confirmation du diagnostic par analyse histologique et immuno-histochimique. Des ponctions pleurale et péricardique ont été effectuées. Le patient a été confié au service d'oncologie pédiatrique pour la chimiothérapie. L’évolution a été marquée par le décès du patient 18 jours après. La manifestation cardiaque est une entité rare et très souvent méconnue.la confirmation histologique est parfois difficile à obtenir du vivant de la plupart des patients.     La biopsie radioguidée constitue une alternative à la biopsie chirurgicale invasive et à la cytologie pour le diagnostic des tumeurs pédiatriques. L'intérêt de notre étude est d’évaluer la valeur diagnostique des biopsies radioguidées examinées au laboratoire d'anatomopathologie de l'hôpital d'Enfants de Rabat (HER). L’étude a porté sur 78 biopsies radioguidées recueillies dans notre laboratoire entre janvier 2008 et décembre 2011. l’âge moyen des patients était de 5 ans et 10 mois avec une prédominance masculine (65,4%). La tumeur était abdominale dans 80% des cas, thoracique dans 15% cas, thoracique et abdominale dans 2,5% et sacrée dans 1,2%. Les biopsies étaient écho-guidées dans 90% des cas et scannoguidées dans 10% des cas. Le diagnostic histopathologique était posé dans 89% des cas. L'immuno-histochimie a été indiquée dans 35% des cas. Les diagnostics les plus fréquents étaient: tumeurs neuroblastiques (42 cas), lymphomes non hodgkiniens (10 cas), rhabdomyosarcomes (6 cas), autres (sarcome d'Ewing, néphroblastomes, tumeur myofibroblastique inflammatoire, maladies de Hodgkin, leucémie aiguë, hépatoblastome et ostéosarcome). Dans notre série, la biopsie radioguidée a permis un diagnostic histopathologique certain dans 89% des cas. Elle nécessite une étroite collaboration entre clinicien, radiologue et anatomopathologiste pour discuter son indication, afin de diminuer le nombre de biopsies peu ou non représentatives.     We report a case of previously undiagnosed Yersinia enterocolitica infection in a 46-year old woman. She consulted her physician because of continual weight loss and physical lassitude. A leucocytosis was found. Sonography revealed an excessive enlargement of abdominal lymph nodes. A malignant lymphoma was suspected and the patient underwent a staging by CT. There the disease was limited on mesenteric and retroperitoneal lymph nodes. Bone marrow biopsy and CT-guided lymph node biopsy did not confirm a systemic lymphatic disease. The patient did not undergo a special therapy. After six months, CT showed a clear regression of enlarged lymph nodes. Finally, a previous Yersinia enterocolitica infection of immunotype 03 could be proved serologically. At this time, the patient had no complaints. Diagnostic and differential diagnosis of benign abdominal lymph node enlargement are discussed based on literature. (orig.) [Deutsch] Berichtet wird der Fall einer klinisch inapperenten Yersinia-enterocolitica-Infektion bei einer 46jaehrigen Patientin, die aufgrund stetigen Gewichtsverlustes und koerperlicher Abgeschlagenheit den Hausarzt konsultierte. Dieser diagnostizierte eine Leukozytose. Die daraufhin durchgefuehrte Sonographie ergab eine massive abdominale Lymphknotenvergroesserung. Unter dem Verdacht eines malignen Lymphoms erfolgte eine computertomographische Ausbreitungsdiagnostik, die die Erkrankung auf mesenteriale und retroperitoneale Lymphknoten beschraenkt zeigte.     Therapieplan. Insgesamt wurden 29 Personen zwecks posttherapeutischem Restaging untersucht. Bei allen wurden Computertomographie- und Lymphknoten-Ultraschalluntersuchungen angewendet. Die Ergebnisse wurden durch den klinischen Verlauf validiert, in drei Faellen wurde ein Rezidiv durch Biopsie bestaetigt. Ergebnisse: CT und US zeigten beim praetherapeutischen Staging 77 Herde verglichen mit 100 durch Doppelkopf-(DK-)PET dargestellten, dieser Unterschied war im Fisher-T-Test jedoch nicht signifikant (p>0,05). Bei der Erkennung residualer Erkrankungsaktivitaet erzielte DK-Koinzidenz-PET eine Sensitivitaet von 93%, eine Spezifitaet von 79%, einen positiven von 82% und einen negativen praediktiven Wert von 92%. Fuer CT+US ergaben sich entsprechend 87%, 64%, 72% und 88%. Schlussfolgerung: FDG-DK-PET erzielt beim Prestaging maligner Lymphome mindestens so genaue Ergebnisse wie ueblicherweise eingesetzte morphologische Diagnoseverfahren. Zusaetzlich ergibt sich ein erkennbarer Vorteil im therapeutischen Monitoring residualer Lymphome durch den Einsatz von Positronen-Koinzidenz-Detektion mittels einer 3/4-Zoll-Kristall-Gammakamera.     Full Text Available Les tumeurs malignes de l'intestin grèle (TMPIG sont des tumeurs rares. Elles représentent 1 à 5% de toutes les tumeurs du tube digestif. Elles sont caractérisées par une hétérogénéité anatomopathologique et une symptomatologie pauvre et non spécifique entrainant ainsi un retard diagnostic, des difficultés Thérapeutiques et donc un mauvais pronostic. Nous rapportant les caractéristiques épidémiologiques, diagnostiques et thérapeutiques ainsi que la survie des patients atteints des TMIG au sein de l’institut national d’oncologie de Rabat. Il s’agit d’une analyse rétrospective des dossiers cliniques des 27 patients ayant le diagnostic de tumeurs malignes de l’intestin grèle admis dans notre institut entre 1998 et 2002. L’âge médian était de 46 ans (15-70 ans. Le délai médian de diagnostic était de 6 mois (0-96 mois. La douleur abdominale était le symptôme le plus fréquent (77.8%. L’étude anatomopathologique a montré 63% de lymphome non Hodgkinie, 14.8% d’adénocarcinome, 7.4% de tumeur stromale, 7.4% de carcinome neuroendocrine et 7.4% de sarcome intestinal. Vingt patients (76. 9% ont eu une résection chirurgicale et 14 patients (53. 8% ont eu une chimiothérapie en fonction du stade de la maladie et du type histologique. Après un recul médian de 11.6 mois, la survie globale après 12 mois était de 44.4% et la médiane de survie était de 11.6 mois. Les tumeurs malignes de l’intestin grèle sont rares.     Lymphocyte-predominant Hodgkin disease (LPHD) is rare and has a natural history different from that of classic Hodgkin disease. There is little information in the literature regarding the role of chemotherapy in patients with early-stage LPHD. The objective of this study was to examine recurrence free survival (RFS), overall survival (OS), and patterns of first recurrence in patients with LPHD who were treated with radiotherapy alone or with chemotherapy followed by radiotherapy. From 1963 to 1996, 48 consecutive patients ages 16-49 years (median, 28 years) with Ann Arbor Stage I (n = 30 patients) or Stage II (n = 18 patients), very favorable (VF; n = 5 patients) or favorable (F; n = 43 patients) LPHD, according to the European Organization for Research and Treatment of Cancer and Groupe d'Etude des Lymphomes de l'Adulte (EORTC-GELA) criteria, received radiotherapy alone (n = 37 patients) or received chemotherapy followed by radiotherapy (n = 11 patients). The percentages of patients with VF disease (11% vs. 9% in the radiotherapy group vs. the chemotherapy plus radiotherapy group, respectively) or F disease (89% vs. 91%, respectively) within the two treatment groups were similar (P = 1.00). A median of three cycles of chemotherapy with mechlorethamine, vincristine, procarbazine, and prednisone (MOPP) or with mitoxantrone, vincristine, vinblastine, and prednisone (NOVP) was given initially to six patients and five patients, respectively. A median total radiotherapy dose of 40 grays (Gy) given in daily fractions of 2.0 Gy was delivered to both treatment groups. The median follow-up was 9.3 years, and 98% of patients were observed for > or = 3.0 years. RFS was similar for patients who were treated with radiotherapy alone and patients who were treated with chemotherapy followed by radiotherapy (10-year survival rates: 77% and 68%, respectively; P = 0.89).     Décrire les caractéristiques épidémiologiques et histo-pathologiques des tumeurs malignes du sein au Cameroun. Il s'agissait d'une étude rétrospective descriptive portant sur les tumeurs malignes du sein, colligées, dans les registres des différents laboratoires d'Anatomie Pathologique publiques et privés repartis dans cinq régions (centre, littoral, Ouest, Nord-ouest, Sud-ouest), pendant une période de 10 ans (2004-2013). Les paramètres étudiés étaient la fréquence, l’âge, le sexe, la localisation, le type et le grade histologique, et les récepteurs hormonaux. Un total de 3044 cas de cancers du sein a été recensé, soit une fréquence annuelle de 304,4 cas en moyenne. Le sexe féminin était le plus représenté avec 2971 cas (97,60%) et les hommes avec 73 cas (2,40%), soit un sexe ratio (H/F) de 0,02. L’âge moyen des patients était de 46±15,87 ans, avec des extrêmes de 13 et 95 ans. Selon la localisation, le sein gauche était atteint dans 1244 cas (52%) et le sein droit dans 1115 cas (47%). Au plan histologique, on retrouvait essentiellement des carcinomes avec 96,50% des cas, des sarcomes 1,39%, des lymphomes 1,07% et la maladie de Paget du mamelon, 1,03%. Les tumeurs épithéliales étaient infiltrantes dans 2049 cas (84,46%), avec une prédominance du carcinome canalaire infiltrant (1870 cas) et non infiltrantes dans 377 cas (15,54%). Le grade histo-pronostic de SBR avait révélé une prédominance du grade II dans 66% des cas. Les cancers du sein restent une pathologie fréquente au Cameroun et atteignent principalement la population féminine en âge de procréer.     Moriz Kaposi was the first who, in 1872, described five patients presenting with `sarcoma idiopathicum multiple hemorrhagicum`. In 1912 Sternberg termed this disease Kaposi`s sarcoma. Since then various forms of this rare disease have been observed. In 1914 Hallenberg described the first cases of African or endemic Kaposi`s sarcoma. In the 1960`s the first reports discussing Kaposi`s sarcoma following organ transplantation and immunosuppressive therapy were published. After 1981, the epidemic form associated with the acquired immunodeficiency syndrome (AIDS) was described. All these forms, their history, treatment methods and the role of radiation therapy in the management of this rare malignancy are discussed, and the literature is reviewed. (authors)

    Retrospective and prospective cohort studies suggest that central nervous system involvement occurs in approximately 0.5 % of patients with advanced Hodgkin's lymphoma. The isolated primary intracranial manifestation of Hodgkin's lymphoma is an extremely rare finding, with few cases reported in the literature. Little is known about the optimal treatment and prognosis of these tumors. Here, we present a case report with a review of the literature. A 47-year-old Caucasian man with persistent frontal headache and unspecific vertigo for half a month was diagnosed with nodular space-occupying lesions in the cerebellum. His medical history included multiple sclerosis, which was treated for 20 years with the immunosuppressive drug azathioprine. Further staging revealed no additional lesions suspected of being malignant. The patient underwent total tumor resection. Immunohistopathological examination showed Epstein-Barr virus-associated classic Hodgkin's lymphoma. Diagnostic bone marrow punction excluded lymphoma involvement of the bone marrow. The patient had no B symptoms. Consequently, the patient was classified as having stage I{sub E}A disease according to the Modified Ann Arbor Classification of Hodgkin Lymphoma and received systemic chemotherapy followed by radiation therapy for the former cerebellar tumor region. He was in complete clinical remission at the last follow-up 9 months after the initial diagnosis. This case report and literature review suggest that multimodal treatment leads to a remarkable clinical outcome in Hodgkin's lymphoma with intracranial involvement. (orig.)     The investigations were carried out on the salivary gland chromosomes of Chironomus tentons. Tritiated compounds (H{sup 3}-thymidine, H{sup 3}-uridine, H{sup 3}-amino-acids), injected into the haemolymph of the larvae should indicate the place of incorporation within the giant chromosomes. After fixation of the salivary glands autoradiographs of the squash-preparations were made. The autoradiographs show that giant chromosomes are most suitable to localize the activity at chromosomal structures with high resolution. DNA-synthesis (thymidine), RNA-synthesis (uridine) and protein-synthesis within the cell could be followed by determining the time and approximatively the quantity of incorporation: Contrary to the protein-synthesis, the DNA-synthesis and the RNA-synthesis are restricted to the chromosomes. The essential physiological activity of the chromosomes seems to be represented by RNA synthesis which takes place at certain distinct loci (nuceolar organizers, ''Balbiani-rings'', puffs, and other chromosomal bands).     With approximately 10000 cases per year in France, non-Hodgkin's lymphoma (NHL) represents the most frequent hematological malignancy, and 5 to 10 % of new cases of cancers. NHLs constitute a heterogeneous group of lympho-proliferative diseases, including entities with very different epidemiological and evolutive characteristics, as well as prognosis and treatments. Several classifications exist, but in practice, we individualize aggressive NHL including Diffuse Large B-Cell Lymphomas (DLBCL) which is the most common lymphoma, and indolent NHL including follicular lymphomas and mucosa-associated lymphoid tissue (MALT) lymphomas. The role of the radiotherapy in the management of NHLs varies according to the specific sub-type of lymphoma, but it has become increasingly limited over time. Overall it finds indications with curative intent only in situations of localized LMNH: either associated with chemotherapy as part of a combined modality therapy as for the treatment of localized DLBCL, or as exclusive treatment specially in the rare situations of localized follicular lymphomas. Moreover, lymphocytes being extremely radiosensitive cells, radiotherapy retains excellent indications with palliative intent for the management of symptomatic bulky tumor masses, and that whatever the sub-type of NHLs may be. It is important to remember that even today the 'Involved Field' irradiation type remains the gold standard for the treatment of nodal NHLs, even if we witness at present the emergence of new types of irradiation, which aim to reduce the amount of irradiated tissues to try to limit the risks of delayed radio-induced complications. The purpose of this article is to clarify the specific aspects (epidemiological, radio-anatomical and prognostic characteristics) of each NHLs'sub-types (except primary central nervous system lymphomas), as well as the practical modalities of the irradiation (illustrated by a clinical case record) when an indication of radiotherapy is placed for their treatment.     Les localisations ORL extra ganglionnaires de la tuberculose sont rares. La symptomatologie clinique ainsi que les examens paracliniques sont souvent trompeurs,posant ainsi le problème de diagnostic différentiel avec la pathologie tumorale. Nous rapportons 15 cas de localisations extra ganglionnaires de tuberculose, colligés au service ORL et CCF de l'Hopital Militaire Avicenne de Marrakech colligés entre 2009 et 2013. L’âge moyen de nos patients est de 33 ans. L’étude topographique a montré 6 cas au niveau du cavum, un cas de miliaire tuberculeuse pharyngée, 4 cas laryngés; 2 localisations auriculaires; 1 parotidienne et 1 localisation sous maxillaire. Le diagnostic était anatomopathologiquedans tous les cas. Tous nos patients ont reçu un traitement antituberculeux avec une bonne évolution.     For 3 decades, GHESKIO (the Groupe Haitien d'Etude du Sarcome de Kaposi et des Infections Opportunistes), the Haitian Ministry of Health, and Weill Cornell have pursued a tripartite mission of service, training, and translational research. The initial focus was on AIDS and tuberculosis. The mission has expanded to include the local community and now provides maternal-child health, family planning, cancer prevention and treatment, immunizations (including human papillomavirus, cholera), and primary education through vocational and microcredit programs. Outcome measures include a reduction in HIV prevalence from 6.2% to the current 2.2%, extensive tuberculosis and cholera prevention and treatment programs, and national training programs for biomedical and community health workers.

    Décrit la première fois par Virchow en 1860, le liposarcome est une tumeur mésenchymateuse rare. Cette rareté est relative car les liposarcomes représentent quand même 14 à 18% de l'ensemble des tumeurs malignes des parties molles et ils constituent le plus fréquent des sarcomes des parties molles. Pour la majorité des auteurs, il ne se développerait jamais sur un lipome ou une lipomatose préexistant. Nous rapportons un cas de volumineux liposarcome de la face dorsale du tronc. L'histoire de la maladie, l'aspect clinique inhabituel « de tumeur dans tumeur », l'aspect de la pièce opératoire nous fait évoquer la possibilité de la transformation maligne d'un lipome bénin préexistant.     The discovery and successful clinical application of the potent anti-tumor compound, cis-Dichlorodiammineplatinum(II), cis-DDP* has stimulated considerable interest in developing effective but less toxic second-generation platinum antitumor drugs. One such candidate drug is cis-Dichloro-trans-dihydroxo-bis-(isopropylamine)platinum(IV), cis-trans-[PtCl 2 (OH) 2 (i-PrNH 2 ) 2 ], (CHIP). An important feature of this Pt(IV) agent is that in addition to exhibiting a generally milder clinical toxicity than cisplatin, the dose-limiting toxicity of CHIP is the more common myelosuppression rather than the less desirable nephrotoxicity. Also, CHIP has been reported recently to be more effective than cisplatin against both alkylating agent sensitive and resistant strains of the Yoshida sarcoms. A microscale synthesis for /sup 195m/Pt-labelled CHIP and the tissue distribution and pharmacokinetic properties of this agent in normal female Fischer 344 rats are reported.     Patients with classical Hodgkin's lymphoma (cHL) have a good prognosis even in advanced stages. However, combined chemo- and radiotherapy, as the standard of care, is also associated with treatment-related toxicities such as organ damage, secondary neoplasias, infertility, or fatigue and long-term fatigue. Many patients suffer from this burden although their cHL was cured. Therefore, the efficacy of immune checkpoint inhibitors like anti-PD1/PD-L1 antibodies in the treatment of solid cancers and also in HL offers new options. A remarkable and durable response rate with a favorable toxicity profile was observed in heavily pretreated cHL patients. Planning to perform prospective randomized clinical trials in the content of radio-immune treatment in patients with Hodgkin's lymphoma (HL), we transferred the results of preliminary clinical studies and basic research in clinical relevant study concepts. Based on these promising early phase trial data, the German Hodgkin Study Group (GHSG) will investigate innovative treatment regimens in upcoming phase II trials. The therapeutic efficacy and potential synergies of anti-PD1 antibodies in combination with chemo- or radiotherapy will be investigated in various settings of HL. (orig.)   2. Intensive combined modality therapy of small round cell and undifferentiated sarcomas in children and young adults

    Seventy-five patients (ages 4-35 years) with the following small round cell tumors and undifferentiated sarcoma were treated at the National Cancer Institute: Ewing's sarcome (n=32), peripheral neuroepithelioma (n=14), rhabdomyosarcoma (n=24), undifferentiated sarcoma (n=5). Most patients had poor prognostic features including 36 (48%) with metastatic disease, and 42 (56%) with central (truncal) tumors (22 in the pelvis). Treatment included 5 cycles of intensive induction chemotherapy with vincristine, cyclophosphamide and adriamycin, plus aggressive local radiation therapy using simulation and computerized treatment planning for all patients. Thereafter, complete clinical responses were consolidated with intensive chemotherapy, total body irradiation and autologous bone marrow transplantation. There were three local only failures, 10 local plus distant failures, 36 distant only failures, 3 treatment-related deaths, and one intercurrent death. Overall actuarial survival and event-free survival at 4 years are 49 and 29%, respectively. Actuarial freedom from local progression was seen in 74% of patients at 4 years, quite remarkable considering the bulk and location of most of these tumors. Without aggressive surgery, many of these high risk patients had satisfactory outcomes, but better systemic treatments are still needed.(author). 44 refs.; 8 figs.; 6 tabs

    The first oral cholera vaccine (OCV) campaign, since its prequalification by the World Health Organization, in response to an ongoing cholera epidemic (reactive vaccination) was successfully conducted in a poor urban slum of approximately 70,000 inhabitants in Port-au-Prince, Haiti, in 2012. Vaccine coverage was 75% of the target population. This report documents the impact of OCV in reducing the number of culture-confirmed cases of cholera admitted to the Groupe Haïtien d'Etude du Sarcome de Kaposi et des Infections Opportunistes (GHESKIO) cholera treatment center from that community in the 37 months postvaccination (April 2012-April 30, 2015). Of 1,788 patients with culture-confirmed cholera, 1,770 (99%) were either from outside the vaccine area (1,400 cases) or from the vaccinated community who had not received OCV (370 cases). Of the 388 people from the catchment area who developed culture-confirmed cholera, 370 occurred among the 17,643 people who had not been vaccinated (2.1%) and the remaining 18 occurred among the 52,357 people (0.034%) who had been vaccinated (P cholera in outbreak settings. © The American Society of Tropical Medicine and Hygiene.

    The homeostasis of insulin-like growth factor-1 (IGF-1) is essential for metabolism, development and survival. Insufficient IGF-1 is associated with poor recovery from wounds whereas excessive IGF-1 contributes to growth of tumours. We have shown that cyclic glycine-proline (cGP), a metabolite of IGF-1, can normalise IGF-1 function by showing its efficacy in improving the recovery from ischemic brain injury in rats and inhibiting the growth of lymphomic tumours in mice. Further investigation in cell culture suggested that cGP promoted the activity of IGF-1 when it was insufficient, but inhibited the activity of IGF-1 when it was excessive. Mathematical modelling revealed that the efficacy of cGP was a modulated IGF-1 effect via changing the binding of IGF-1 to its binding proteins, which dynamically regulates the balance between bioavailable and non-bioavailable IGF-1. Our data reveal a novel mechanism of auto-regulation of IGF-1, which has physiological and pathophysiological consequences and potential pharmacological utility.

    Purpose. - Surgery is the treatment of choice for localized uterine sarcomas. We conducted a retrospective study to define prognostic factors. Patients and methods. - We studied 111 cases of patients treated by adjuvant radiotherapy for uterine sarcoma in seven French centers. The median decline was 31 months. We conducted a univariate analysis to identify factors correlated with local recurrence. The statistically significant factors were studied in multivariate analysis by Cox model. Results. - The median dose of external beam radiotherapy was 45 Gy. Forty-three percent of patients had vaginal vault brachytherapy and 21 % chemotherapy. Only 6.3 % of patients had complications of acute grade III and 8.1 % of long-term sequelae of radiotherapy. The survival rate at 5 years was 74.6 %. They noted 12.6 % of isolated locoregional recurrences, against 29.7 % for distant recurrences, 80 % were pulmonary. Factors correlated with the risk of locoregional relapse were menopausal status (P = 0.045) and surgical margins suspicious or not healthy (P = 0.0095). The chemotherapy did not improve overall survival or disease free survival but the numbers were low. Conclusion. - The postoperative radiotherapy provides good local control in this disease. Brachytherapy is sometimes done, but it does not improve local control. Chemotherapy is not a standard localized stage but the rate of metastatic recurrence calls for the development of strategies involving systemic treatment with radiotherapy. (authors)

    Cardiomyocyte loss in the ischemically injured human heart often leads to irreversible defects in cardiac function. Recently, cellular cardiomyoplasty with mesenchymal stem cells, which are multipotent cells with the ability to differentiate into specialized cells under appropriate stimuli, has emerged as a new approach for repairing damaged myocardium. In the present study, the potential of human umbilical cord-derived mesenchymal stem cells to differentiate into cells with characteristics of cardiomyocyte was investigated. Mesenchymal stem cells were isolated from endothelial/subendothelial layers of the human umbilical cords using a method similar to that of human umbilical vein endothelial cell isolation. Isolated cells were characterized by transdifferentiation ability to adipocytes and osteoblasts, and also with flow cytometry analysis. After treatment with 5-azacytidine, the human umbilical cord-derived mesenchymal stem cells were morphologically transformed into cardiomyocyte-like cells and expressed cardiac differentiation markers. During the differentiation, cells were monitored by a phase contrast microscope and their morphological changes were demonstrated. Immunostaining of the differentiated cells for sarcomeric myosin (MF20), desmin, cardiac troponin I, and sarcomeric α-actinin was positive. RT-PCR analysis showed that these differentiated cells express cardiac-specific genes. Transmission electron microscopy revealed a cardiomyocyte-like ultrastructure and typical sarcomers. These observations confirm that human umbilical cord-derived mesenchymal stem cells can be chemically transformed into cardiomyocytes and can be considered as a source of cells for cellular cardiomyoplasty

    This contribution outlines possibilities and limitations of whole-body MRI for investigating musculoskeletal diseases. Benefits and drawbacks of the novel whole-body MRI technology are discussed and a possible whole-body MRI sequence protocol for musculoskeletal examinations is proposed. Muscle, joint and bone diseases are discussed in which the application of whole-body MRI may be of advantage. Particularly, polymyositis, muscledystrophy, rheumatoid arthritis, spondylitis ancylosans, multiple trauma, skeletal metastases, multiple myeloma and malignant lymphoma are mentioned. Whole-body MRI opens new advantages for the examination of multifocal musculoskeletal diseases. The clinical benefit of this method for particular diseases has to be evaluated in further studies, however. (orig.) After review, with initial diagnosis of anaplastic carcinoma versus lymphoma, immunocytochemical study was performed. Smears were positive for B cell markers and negative for cytokeratin, so with the impression of diffuse large B cell lymphoma, the patient received two courses of chemotherapy by which the tumor disappeared during two weaks. Despite previous reports, stating easy diagnosis of high-grade thyroid lymphoma on the grounds of cytomorphological features we like to emphasize, overlapping cytologic features of the curable high grade thyroid lymphoma form noncurable anaplastic thyroid carcinoma and usefulness of immunocytochemistry to differentiate these two disease.

    For the follicular lymphomas, the PET is not in the systematic medical evaluation when it cannot be ignored in the evaluation of high grade non hodgkin lymphomas because it allows to make a complete extension evaluation and especially to help to define the target volume when the radiotherapy is indicated. The fixation of F.D.G. in follicular lymphomas was studied. We showed that the follicular lymphomas present in PET under the form of hypermetabolic injuries. It seems possible to consider the use of PET to help at the definition of target volumes when the radiotherapy is indicated. The intensity of fixation seems to have a predictive value for the therapy response in case of immunotherapy. The measurement of the active tumoral volume seems to have a predictive value for the response to the treatment that is worth being specified on a more important population. (N.C.)

    Full Text Available Abstract Background Flatfish metamorphosis is a thyroid hormone (TH driven process which leads to a dramatic change from a symmetrical larva to an asymmetrical juvenile. The effect of THs on muscle and in particular muscle sarcomer protein genes is largely unexplored in fish. The change in Troponin T (TnT, a pivotal protein in the assembly of skeletal muscles sarcomeres and a modulator of calcium driven muscle contraction, during flatfish metamophosis is studied. Results In the present study five cDNAs for halibut TnT genes were cloned; three were splice variants arising from a single fast TnT (fTnT gene; a fourth encoded a novel teleost specific fTnT-like cDNA (AfTnT expressed exclusively in slow muscle and the fifth encoded the teleost specific sTnT2. THs modified the expression of halibut fTnT isoforms which changed from predominantly basic to acidic isoforms during natural and T4 induced metamorphosis. In contrast, expression of red muscle specific genes, AfTnT and sTnT2, did not change during natural metamorphosis or after T4 treatment. Prior to and after metamorphosis no change in the dorso-ventral symmetry or temporal-spatial expression pattern of TnT genes and muscle fibre organization occurred in halibut musculature. Conclusion Muscle organisation in halibut remains symmetrical even after metamorphosis suggesting TH driven changes are associated with molecular adaptations. We hypothesize that species specific differences in TnT gene expression in teleosts underlies different larval muscle developmental programs which better adapts them to the specific ecological constraints.

    The place of positron emission tomography (PET) in the evaluation of diffuse at big cells B lymphomas and hodgkin lymphomas is validated. The clinical impact of the PET registered in end of therapy development is indisputable. recommendations must be followed for images interpretation. The PET is strongly recommended during the first evaluation of the disease because it is a reference examination that makes easy the interpretation at the end of the therapy and allows to evaluate the extension of the disease with a sensitivity and a specificity superior to the computerized tomography. the prognosis value of intermediate evaluations appears certain in the diffuse at big cells B lymphomas and the hodgkin lymphomas but the impact of an early therapy change induced by PET is still to determine. The criteria of interpretation of early evaluations are to standardize. for the other types of lymphomas, the PET can have an interest to confirm the the localized stages, especially for the follicular lymphomas and direct the biopsy for a patient ill of a low range lymphoma suspect of aggressive change.     Clinical, hematological and osseous effects following an intramuscular injection of 100 {mu}Ci 90 Sr were studied in the rat. In spite of the magnitude of the injection and the resulting damage, the elimination of strontium could compare with what occurs after an injection at tracer doses. Comparing with the controls at the outcoming time, clinical monitoring mainly brought out a loss of body weight. Fairly early (20. - 30. day) there occurred severe hematological damage, especially on lymphocyte line, which subsided spontaneously. As foreseen, the anatomo-pathological survey of the early period showed bone and medullar lesions in the areas of enchondral ossification. In the late period, bone sarcomas occurred in nine animals out of ten. The outstanding histological type was osteogenic osteosarcomas; besides, two animals experienced bilateral tumors.   18. L’exposition contrôlée à la lumière et à l’obscurité ajuste le rythme du cortisol salivaire chez les travailleurs de nuit

    Full Text Available Dans cette étude réalisée sur le terrain et en laboratoire, nous avons testé l’efficacité d’une intervention consistant à contrôler l’exposition à la lumière/obscurité de façon à favoriser l’adaptation circadienne au travail de nuit. Six travailleurs permanents de nuit (moyenne d’âge 37,1 ± σ : 8,1 ans avaient comme consigne de s’exposer de façon intermittente à une lumière blanche vive plein spectre (~2 000 lux au cours des 6 premières heures de leur quart de travail de 8 heures. Il leur était demandé de se protéger de la lumière solaire du matin avec des verres teintés (densité gris neutre, transmission visuelle de la lumière 15 % et de maintenir des épisodes réguliers de sommeil/obscurité dans une pièce très sombre. Ces épisodes devaient commencer 2 heures après la fin de chaque quart de travail de nuit. Cinq travailleurs du groupe contrôle (41,1 ± σ : 9,9 ans ont été étudiés alors qu’ils devaient uniquement maintenir un horaire régulier de sommeil/obscurité. Des procédures de routine constante, réalisées avant et après une série d’environ 12 quarts de travail de nuit s’échelonnant sur 3 semaines, ont montré chez les travailleurs du groupe traitement une variation significative dans l’heure d’apparition du pic de cortisol ainsi qu’un ajustement de ce rythme à un horaire de travail de nuit. Un plus petit changement de phase a été observé dans le groupe contrôle, ce qui suggère une adaptation partielle à l’horaire de travail de nuit. Nos résultats démontrent l’efficacité d’une exposition judicieuse à la lumière et à l’obscurité pour favoriser l’adaptation des rythmes physiologiques au travail de nuit.The efficacy of a light/darkness intervention designed to promote circadian adaptation to night shift work was tested in this combined field and laboratory study. Six permanent night shift workers (mean age: 37.1 / s: 8.1 years were instructed to expose themselves intermittently to full-spectrum bright white light (~2,000 lux in the first 6 hours of their 8-hour shift. They were asked to shield themselves from morning sunlight with tinted lenses (neutral grey density, 15% visual light transmission, and to maintain regular sleep/darkness episodes in darkened quarters beginning 2 hours after the end of each night shift. Five control group workers (41.1 / s: 9.9 years were observed in the presence of a regular sleep/darkness schedule only. Constant routines performed before and after a sequence of ~12 night shifts over 3 weeks revealed that treatment group workers displayed significant shifts in the time of peak cortisol expression and a realignment of the rhythm with the night-oriented schedule. Smaller phase shifts suggesting an incomplete adaptation to the shift work schedule were observed in the control group. Our observations support a careful control of the pattern of light and darkness exposure for the adaptation of physiological rhythms to night shift work.

    As muco-epidermoid carcinomas are the most frequent tumours of salivary glands for child, the authors report the study of the role of radiotherapy. This study is based on a survey performed in 34 paediatric departments. The authors analyse the age and gender, the use of irradiation, the tumour location, the tumour grade and size, the use of surgery and of radiotherapy, and survival.     The role of radiotherapy in the treatment of oral Kaposi's sarcoma needs to be defined. Over the past six years, 400 patients with AIDS-related epidemic Kaposi's sarcoma were referred to the oncology department of Henri Mondor Hospital; about 50% had oral Kaposi's lesions. Only 29 of the latter group had received oral cavity irradiation; palliation of pain was the most common indication. The age range was from 26-46 yr with a mean of 36 yr. Twenty-five patients were treated by megavolt age irradiation (4 MV X-rays) 22 of whom received 15 Gy (2 Gy/fraction, 8 Gy/week) and three 30 Gy (15 Gy followed by 3 weeks rest, then a further series of 15 Gy). Five patients were treated by orthovoltage irradiation (45 kV) and received 10-20 Gy (one patient received the two methods of irradiation). Treatment was generally successful in achieving palliation: of the 22 patients who received 15 Gy, 13.6% achieved complete remission and 86.4% partial remission with disappearance of pain; however, toxicity related to severe mucositis, even at relatively low doses and in spite of anti-mycotic treatment, is an argument for restrictive use of this modality.

    Significant progress has been achieved during the last years regarding therapy of neoplastic and non-neoplastic diseases of the hematopoietic system by introduction of new therapeutic modalities like highdose chemotherapy, bone marrow and stem cell transplantation, interferon-therapy and others. Diagnosis is still based on biopsy and histopathology of bone marrow. Imaging methods, however, provided by radiology and nuclear medicine, are now increasingly employed to give an additional macroscopic view over morphological and functional changes of the entire bone marrow. Bone marrow scintigraphy either using radiocolloids or immunoscintigraphy against granulocyte-antigenes may be performed as an alternative or an addition to nuclear magnetic resonance imaging. Bone scintigraphy has been successful in the detection of additional bony lesions for more than two decades. Positron emission tomography using 18-fluorine-deoxyglucose has recently been employed as a new and promising tool also for assessment of bone marrow infiltration in malignant lymphomas. (orig.)     Peripheral T-cell lymphomas (PTCLs) are rare and have a dismal prognosis. The most frequent subtype is PTCL, unspecified. Lymph node samples from 110 patients with PTCL, unspecified included in LNH87 and LNH93 trials were available. EBV status was studied by EBV-encoded small RNA in situ hybridization (EBER-ISH). EBER-ISH showed positive cells in 45 (41%) of 110 patients. Pretreatment characteristics were comparable between positive and negative cases, except for male sex (80% versus 60%, respectively, P = .02). Only 50% of patients achieved complete remission with a 5-year event-free survival (EFS) and overall survival (OS) of 21% and 30%, respectively. EBER-ISH positivity was the sole factor linked with worse EFS, with a 5-year probability of 11% for positive patients. In univariate analysis, factors affecting OS were EBER-ISH positivity, high LDH level, and age older than 60 years. In multivariate analysis, EBER-ISH was associated with a worse OS in the elderly population. Time-dependent analysis showed that the negative impact of EBV was essentially seen in the first 2 years following diagnosis. These results warrant further studies regarding pathogenesis and specific treatment approaches for EBV-associated PTCL patients.

    There is a wide variety of spinal extradural tumors. In addition to real neoplasms, degenerative diseases, congenital abnormalities and inflammatory disorders can be causes of extradural masses. Due to the bony boundary of the spinal canal, both benign as well as malignant masses can cause progressive neurological deficits including paraplegia. Most of the spinal tumors are benign (hemangioma of the vertebral body, degenerative diseases). In younger patients congenital abnormalities and primary tumors of the spine have to be considered, whereas in adults the list of differential diagnoses should include secondary malignancies such as metastases and lymphomas as well as metabolic disorders such as osteoporotic vertebral compression fracture and Paget's disease. Cross-sectional imaging techniques such as magnetic resonance imaging (MRI) and computed tomography (CT) of the spine often help to make a specific diagnosis of extradural spinal lesions and represent important tools for tumor staging and preoperative evaluation. Neben echten Neoplasien koennen auch degenerative Veraenderungen und Anlagestoerungen sowie entzuendliche Veraenderungen Ursachen einer extraduralen Raumforderung sein.     From June 1986 to December 1996, 643 patients presenting with acquired immunodeficiency syndrome (AIDS)-related epidemic Kaposi`s sarcoma were treated with irradiation at the Oncology Department of Henri Mondor University Hospital. Three-hundred eighty-seven patients (60 %) had previously received a treatment with interferon (259 patients, 40.2 %), vinblastine (225 patients, 34.5 %), doxorubicin (22 patients, 3.4 %), bleomycin (19 patients, 2.9 %), and/or antiviral treatment (216, 33.5 %). The radiotherapy was delivered by 4 MeV OR 8 MeV electron beam for extended cutaneous fields and 45-100 kV x-ray for localized fields. The delivered dose was 20 Gy in 2 weeks (2.5 Gy/fraction, 4 fractions/week) followed by 2 weeks rest and second series of 10 Gy in 1 week. For oral cavity lesions, we used a series of 15.2 Gy was delivered (1.9 Gy/fraction, 4 fractions/week), followed for three patients by a 3 week rest and by a similar second series of 15.2 Gy.Six-hundred and twenty-one patients were evaluable and the objective response rate was 92 %, with e complete regression of clinical and functional symptoms for all patients. The skin tolerance was good, with 7.3 % grade I reactions, 69.3 % of grade II reactions, and 23.4 % grade III reactions. There was a correlation between recurrence rate and the occurrence of opportunistic infections. This analysis shows the efficacy of dose radiotherapy for treatment of epidemic Kaposi sarcoma.     The role of radiotherapy in the treatment of oral Kaposi's sarcoma needs to be defined. Over the past six years, 400 patients with AIDS-related epidemic Kaposi's sarcoma were referred to the oncology department of Henri Mondor Hospital; about 50% had oral Kaposi's lesions. Only 29 of the latter group had received oral cavity irradiation; palliation of pain was the most common indication. The age range was from 26-46 yr with a mean of 36 yr. Twenty-five patients were treated by megavolt age irradiation (4 MV X-rays) 22 of whom received 15 Gy (2 Gy/fraction, 8 Gy/week) and three 30 Gy (15 Gy followed by 3 weeks rest, then a further series of 15 Gy). Five patients were treated by orthovoltage irradiation (45 kV) and received 10-20 Gy (one patient received the two methods of irradiation). Treatment was generally successful in achieving palliation: of the 22 patients who received 15 Gy, 13.6% achieved complete remission and 86.4% partial remission with disappearance of pain; however, toxicity related to severe mucositis, even at relatively low doses and in spite of anti-mycotic treatment, is an argument for restrictive use of this modality.

    The authors report the study of dose escalation for the treatment of spinal in two types of Ewing tumours. They used 5 dose levels at the rate of five 1,6 Gy per week. They compare different radiotherapy techniques: three-dimensional conformation radiotherapy, static intensity-modulated conformational radiotherapy, helical tomo-therapy, volume-modulated arc-therapy (VMAT), stereotactic radiotherapy, and proton-therapy (in passive diffusion). It appears that it is possible to safely and efficiently deliver until 70,4 Gy in some Ewing tumours. In child, exclusive radiotherapy might become a local treatment option and would require a clinic trial and comparison with exclusive surgery or post-operative radiotherapy. Short communication

    Full Text Available FUNDAMENTO: O potencial de renovação e proliferação dos cardiomiócitos, in vivo, é pequeno, e por isso, o músculo cardíaco apresenta limitada capacidade de repor células perdidas. Na tentativa de minimizar os danos oriundos de lesões hipóxico-isquêmicas e daquelas que acometem o sistema de condução do coração, a terapia celular com células-tronco mesenquimais (MSC vem sendo utilizada, inclusive com cardiomiócitos diferenciados a partir de MSC. OBJETIVO: O presente trabalho comparou três protocolos distintos de indução de diferenciação objetivando a sugestão de um método viável para a diferenciação de maior número de células funcionais que expressem fenótipo cardiomiogênico. MÉTODOS: Culturas de MSC obtidas de tecido adiposo de ratos jovens da linhagem Lewis transgênicos para proteína verde fluorescente (GFP foram submetidos a três diferentes meios de diferenciação cardiogênica: Planat-Bérnard, 5-azacitidina e meio Planat-Bérnard + 5-azacitidina e observadas quanto a expressão de marcadores celulares cardíacos. RESULTADOS: Nos três protolocos utilizados observou-se formação da proteína alfa-actinina sarcomérica no citoesqueleto das células submetidas à diferenciação, expressão de conexina 43 na membrana nuclear e citoplasmática e formação de gap junctions, necessárias para a propagação do impulso elétrico no miocárdio, contudo, em nenhum protocolo foi observada contração espontânea das células submetidas à diferenciação cardiogênica. CONCLUSÃO: A indução com 5-azacitidina proporcionou diferenciação celular cadiomiogênica efetiva e similar à encontrada com o meio Planat-Bénard e, por ser um protocolo mais simples, rápido e com menor custo torna-se o método de eleição.

    Karzinommetastase oder ein primaer zerebrales Non-Hodgkin-Lymphom, gelegentlich auch eine ischaemische Hirngewebsnekrose oder ein Abszess vermutet wird. Diese Annahmen werden durch die subkortikale Lokalisation und die marginal betonte Anreicherung von Kontrastmittel unterstuetzt. Der bioptische Nachweis einer relativ frischen Markschaedigung und einer lymphozytaeren Entzuendungsreaktion bei relativer Unversehrtheit axonaler Strukturen macht die Diagnose einer akuten primaeren Entmarkungserkrankung wahrscheinlich. Die floride reaktive Astrogliose mit z.T. pleomorphen tumorverdaechtigen Reizformen der Astroglia kann so ausgepraegt sein, dass im Rahmen der intraoperativen Schnelldiagnostik ein Glioma in Erwaegung gezogen wird. Exemplarisch werden Befunde einer Serie 14 radiologisch und stereotaktisch-bioptisch dokumentierter ADEM vorgestellt und diskutiert. (orig.)

  12. Les déterminants du statut “perdu de vue” chez les patients pris en charge pour cancer au Maroc: situation avant le Plan Cancer

    Introduction Le cancer au Maroc représente un problème majeur de santé publique, sa prise en charge doit être globale, active et complète pour tous les patients. L'objectif de ce travail était d'estimer la fréquence des perdus de vue « PDV » en oncologie au Maroc durant la première année de suivi et de déterminer les facteurs associés à ce problème. Méthodes Par une étude rétrospective portant sur 2854 dossiers de malades hospitalisés dans les trois principaux centres d'oncologie au Maroc depuis janvier 2003 jusqu’à juin 2007 et concernant les cinq principales localisations de cancer au Maroc, nous avons cherché la date des dernières nouvelles des patients ayant un recul de 18 mois minimum afin de déterminer le statut de ces malades après un an de suivi. Résultats La moyenne d’âge était de 52±14 ans, une proportion féminine de 63%, les sujets actifs constituaient 28%, les mariés 71%, les analphabètes 51%, 70% des patients habitaient en milieu urbain et seulement 11% des malades disposaient d'une couverture sociale. La localisation cancéreuse la plus fréquente était le poumon (23,8%) suivie du colon-rectum (23,5%) puis le col (21,9%), le sein (20,4%) et les lymphomes (10,4%). Le taux des «PDV» à un an de suivi était de 48%, ce statut était significativement lié au sexe, à l’âge, au NSE et au statut matrimonial. Sur le plan médical, le statut «PDV» était lié à la localisation du cancer, au stade de diagnostic et au type de traitement reçu. Conclusion Notre étude a mis en évidence la grande ampleur du problème des PDV en cancérologie au Maroc ainsi que ces déterminants. Ces résultats incitent tous les acteurs dans le domaine de la cancérologie à collaborer ensemble pour prendre les mesures qui s'imposent pour y pallier PMID:25400850

    Field design changed substantially from extended-field RT (EF-RT) to involved-field RT (IF-RT) and now to involved-node RT (IN-RT) and involved-site RT (IS-RT) as well as treatment techniques in radiotherapy (RT) of Hodgkin's lymphoma (HL). The purpose of this article is to demonstrate the establishment of a quality assurance program (QAP) including modern RT techniques and field designs within the German Hodgkin Study Group (GHSG). In the era of modern conformal RT, this QAP had to be fundamentally adapted and a new evaluation process has been intensively discussed by the radiotherapeutic expert panel of the GHSG. The expert panel developed guidelines and criteria to analyse ''modern'' field designs and treatment techniques. This work is based on a dataset of 11 patients treated within the sixth study generation (HD16-17). To develop a QAP of ''modern RT'', the expert panel defined criteria for analysing current RT procedures. The consensus of a modified QAP in ongoing and future trials is presented. With this schedule, the QAP of the GHSG could serve as a model for other study groups. (orig.)     Despite complex treatment schedules for cancer, the occurrence of resistances and relapses is a major concern in oncology. Hence, novel treatment options are needed. In this thesis, different approaches using radioimmunotherapy and the opioid D,L-methadone alone or in combination with doxorubicin were analyzed regarding their cytotoxic potential and the triggered signalling pathways in sensitive and resistant leukaemia and non-Hodgkin lymphoma (NHL). The radioimmunoconjugates [Bi-213]anti-CD33 and [Bi-213]anti-CD20 for treatment of acute myeloid leukaemia (AML) or NHL, respectively, were applied exemplary for the use of targeted alpha-therapies (TAT). Depending on the analyzed cell lines, the used activity concentrations and specific activities (MBq/μg antibody) apoptosis was induced abrogating radio- and chemo-cross-resistances specifically. The cell death was caspase-dependent activating the mitochondrial pathway and was executed by downregulation of the anti-apoptotic proteins XIAP and Bcl-xL. D,L-Methadone induces apoptosis in vitro and in vivo in opioid-receptor (OR) expressing cells depending on the OR density and the used concentrations. Resistances could be overcome and proliferation was inhibited. In combination with doxorubicin, a synergistic effect regarding cytotoxicity in ex vivo patient cells and cell lines was observed. This effect depends on the increase of doxorubicin uptake co-administering D,L-methadone whereas doxorubicin enhances OR expression. The activation of OR leads to the downregulation of cAMP playing a pivotal role in apoptosis induction. In vivo, the therapeutic potential of D,L-methadone alone or in combination with doxorubicin could be proven as mice transplanted with human T-ALL-cells could be identified as tumour free. In summary, these studies show that TAT using [Bi-213]anti-CD33 and [Bi-213]anti-CD20 as well as the opioid D,L-methadone harbour the potential to optimize conventional treatment modalities for leukaemia and NHL.

  16. Apport diagnostique de la cervicotomie exploratrice: étude rétrospective de 300 cas

    Les tuméfactions cervicales représentent un motif fréquent de consultation, et les adénopathies en constituent l’étiologie la plus fréquente. L'examen clinique et les bilans paracliniques permettent, dans la majorité des cas de retrouver une étiologie. Néanmoins certaines de ces tuméfactions restent d'origine non précisée, portant donc l'indication d'une cervicotomie exploratrice. Il s'agit d'une étude rétrospective d'une série de 300 cas de tuméfactions cervicales isolées colligées au service d'ORL de l'hôpital militaire Avicenne de Marrakech entre 2001 et 2014. Tous nos patients ont bénéficié d'une cervicotomie exploratrice avec étude anatomo-pathologique. L’âge des patients varie entre 1 et 76 ans avec un âge moyen de 32,57 ans et une légère prédominance masculine de 52%. La symptomatologie qui a motivé une consultation chez 81% des patients était la tuméfaction latérocervicale. La localisation la plus fréquente était sous mandibulaire (33,34%). Les tuméfactions d'installation progressive ont été retrouvées chez 93,34% des patients. Les principales étiologies retrouvées dans notre étude après examen anatomopathologique étaient de deux types: soit d'origine ganglionnaire dominées par la tuberculose ganglionnaire cervicale (53,66%), le lymphome malin non hodgkinien (6,66%), les adénites réactionnelles non spécifiques (4,66%), la maladie deHodgkin (4,33%) et les métastases ganglionnaires cervicales (3,33%); soit d'origine non ganglionnaire dont le lipome cervicale (17,66%), les kystes branchiaux (6%), les kystes du tractus thyréoglosse (1,66%) et le lymphangiome kystique (1,66%).     We report the case of a 19-year-old young woman for whom was discovered a pregnancy at the end of the irradiation for a Hodgkin lymphoma (stage IV bone Bb), initially treated by chemotherapy. The radiotherapy delivered 36 Gy in infra-diaphragmatic volumes (lumbo-aortic, spleen, L5), beginning in a pregnant patient for less than 4 amenorrhea weeks. The calculated received fetal dose (literature data, measurement with software T.P.S., measurement on phantom) is high: it's between 2.8 and 5 Gy. With a current follow-up of 4 years since the radiotherapy's end, the patient is in complete remission and her child presents with a normal development for the age, in spite of the infra diaphragmatic irradiation.     Four years after the irradiation, the patient was in situation of complete forgiveness: the child was normal, there was neither a deformation nor a mental disorder, but the neoplastic risk persists, thus long-term surveillance is imperative. If the pregnancy is known, we have to arrest the foetal risks incurred in order to propose a termination of pregnancy; therefore, are made the measures on phantoms of the dose that he would receive, according to the tumoral localization, to the dose necessary to eradicate the tumor, to the field and the possible masks for an optimal tumoral control.     The authors report the assessment of low-dose radiotherapy (two sessions of 2 Gy in two days) for the curative treatment of mucosa-associated lymphoid tissue (MALT) lymphoma of the lung. The treatment of this lymphoma is discussed in terms of surgery, chemotherapy, radiotherapy, or even simple monitoring.     -jaehrigen Patienten mit einem zentrozytisch-zentroblastischen Non-Hodgkin-Lymphom, Stadium IA (Lokalisation: Linke Leiste) wurde die gesamte Abdomen- und Beckenregion ('abdominelles Bad') bestrahlt. Bei einer woechentlichen Fraktionierung von fuenfmal 1,5 Gy wurde eine Gesamtdosis von 30 Gy appliziert. Zum Schutz der Risikoorgane wurden Nierenbloecke nach 13,5 Gy und Leberbloecke nach 25 Gy eingesetzt. Waehrend der letzten beiden Therapietage kam es zur Verschlechterung des Allgemeinzustandes des Patienten mit Gewichtszunahme, Vergroesserung des Bauchumfanges, Dyspnoe und einem Anstieg der Leberwerte. Die weiterfuehrende Diagnostik ergab eine Hepatosplenomegalie mit ausgepraegter Aszitesbildung und einen erhoehten portosystemischen Druckgradienten. Im Leberbiopsat wurde eine Venenverschlusskrankheit gefunden. Innerhalb 1 Woche nach Anlage eines transjungulaeren intrahepatischen portosystemischen Stent-Shunts (TIPSS), Vollheparinisierung und unter Diuretikagabe war der Patient beschwerdefrei. Die Leberwerte sind im Normbereich. Schlussfolgerung: Die Venenverschlusskrankheit der Leber (VOD) ist eine sehr seltene Nebenwirkung bei der abdominellen Bestrahlung nicht vorbehandelter follikulaerer Keimzentrumslymphome. Bei Oberbauchbeschwerden, Anstieg der Leberenzyme sowie Aszitesbildung, insbesondere in einem Zeitraum von bis zu 4 Monaten nach Therapieabschluss, muss an diese Komplikation gedacht werden.     Objective. - The aim of this prospective study is the feasibility of late effects assessment by LENT-SOMA scales after conservative treatment of soft tissue sarcomas of the extremities and a comparison with the functional evaluation by the Enneking score. Patients and methods. - During the systematic follow-up consultations, a series of 32 consecutive patients was evaluated in terms of late effects by LENT SOMA scales and functional results by the Enneking score. The median time after treatment was 65 months. The treatment consisted of conservative surgery (all cases) followed by radiation therapy (29 cases), often combined with adjuvant therapy (12 concomitant radio-chemotherapy association cases out of 14). The assessment of the toxicity was retrospective for acute effects and prospective for the following late tissue damage: skin/subcutaneous tissues, muscles/soft tissues and peripheral nerves. Results. -According to the Enneking score, the global score for the overall series was high (24/30) despite four the scores zero for the psychological acceptance. According to LENT SOMA scales, a low rate of severe sequelae (grade 3-4) was observed. The occurrence of high-grade sequelae and their functional consequences were not correlated with quality of exeresis, dose of radiotherapy or use of concomitant chemotherapy. A complementarity was observed between certain factors of the Enneking score and some criteria of the LENTSOMA scales, especially of muscles/soft tissues. Conclusion. -The good quality of functional results was confirmed by the two mean scoring systems for late normal tissue damage. The routine use of LENT-SOMA seems to be more time consuming than the Enneking score (mean time of scoring: 1 3 versus five minutes). The LENT-SOMA scales are aimed at a detailed description of late toxicity and sequelae while the Enneking score provides a more global evaluation, including the psychological acceptance of treatment. The late effects assessment by the LENT-SOMA scales should be carried on in prospectives studies, especially in case of concomitant radio-chemotherapy. (author)

    The authors report the assessment of the local control, global survival and survival without recurrence, and also late complications, based on retrospective study over 8 years of 169 cases Hodgkin lymphoma in patients older than 18 (average age of 34) and who had been submitted to radiotherapy. Survival has been computed according to the Kaplan-Meier method. The authors notice that the recurrence rate is slightly higher than in the literature. During the considered period, radiotherapy was two-dimensional with higher doses than those corresponding to current standards. The improvement of radiotherapy techniques and the decrease of doses within the standards should improve the therapeutic results. Short communication

Aufgrund dessen erfolgte eine Aktualisierung der Leitlinien der DEGRO/DGK zur Strahlentherapie dieses Patientenkollektivs. Wir berichten von einem Patienten mit ICD-Implantat und kardialer Beteiligung eines T-lymphoblastischen Lymphoms, der u. a. eine Ganzkoerperbestrahlung mit 8 Gy und eine anschliessende konsolidierende Bestrahlung des Perikards mit 14 Gy sowie erneute Bestrahlungen in Rezidivsituation erhalten hat. Im Rahmen der Behandlung wurde die antitachyarrhythmische (ATA) Therapie ausgeschaltet und voruebergehend durch eine LifeVest ersetzt. Gemaess der aktuellen DEGRO-Leitlinie zur Bestrahlung von Patienten mit kardialen implantierten elektronischen Geraeten erfolgte die Einstufung des Patienten in die ''High-risk''-Gruppe. Ausserdem wurden regelmaessige telemetrische Abfragen des ICD-Geraets vor und nach Behandlung durchgefuehrt. Trotz fehlender Angabe des Herstellers bezueglich der erlaubten Kumulativdosis und Empfehlung der DEGRO ueber eine maximale Gesamtdosis <2 Gy, zeigte sich bei einer Gesamtdosis im Bereich des Geraets von >10 Gy in der ICD-Abfrage nach abgeschlossener Strahlentherapie eine unauffaellige Nachsorge mit regelrechten Messwerten fuer alle relevanten ICD-Parameter. Der Fall zeigt, dass bei Vermeidung einer direkten Bestrahlung der kardial implantierten elektronischen Geraete und bei Verwendung einer Energie von maximal 6 MV, eine strahleninduzierte Schaedigung weniger wahrscheinlich ist und vermieden werden koennte.     The present study addresses the role of intensity-modulated radiotherapy (IMRT) in contrast to standard RT (APPA) for patients with Hodgkin's lymphoma (HL) with a focus on deep inspiration breath-hold (DIBH) technique and a comparison between the International Lymphoma Radiation Oncology Group (ILROG) Involved Site Radiotherapy (IS-RT) versus the German Hodgkin Study Group (GHSG) Involved Field Radiotherapy (IF-RT). APPA treatment and 2 IMRT plans were compared for 11 patients with HL. Furthermore, treatment with DIBH versus free breathing (FB) and two different treatment volumes, i.e. IF-RT versus IS-RT, were compared. IMRT was planned as a sliding-window technique with 5 and 7 beam angles. For each patient 12 different treatment plans were calculated (132 plans). Following organs at risk (OAR) were analysed: lung, heart, spinal cord, oesophagus, female breast and skin. Comparisons of the different values with regard to dose-volume histograms (DVH), conformity and homogeneity indices were made. IS-RT reduces treatment volumes. With respect to the planning target volume (PTV), IMRT achieves better conformity but the same homogeneity. Regarding the D{sub mean} for the lung, IMRT shows increased doses, while RT in DIBH reduces doses. The IMRT shows improved values for D{sub max} concerning the spinal cord, whereas the APPA shows an improved D{sub mean} of the lung and the female breast. IS-RT reduces treatment volumes. Intensity-modulated radiotherapy shows advantages in the conformity. Treatment in DIBH also reduces the dose applied to the lungs and the heart. (orig.)     Form von ''Kamptokormie'' und ''Dropped Head Syndrom'' (DHS) verstaerkt wissenschaftliche Beachtung gefunden. Die Kamptokormie ist durch zunehmende Flexion des Oberkoerpers beim Gehen bei wenig beeintraechtigtem aufrechtem Stand charakterisiert. Das DHS imponiert durch eine Schwaeche der Nackenmuskulatur, die zu erschwerter Kopfhebung fuehrt. In schweren Faellen ist bei der Kamptokormie das Gehen stark beeintraechtigt und beim DHS das Kinn auf der Brust nahezu fixiert. Viele Erkrankungen koennen eine Kamptokormie verursachen, insbesondere neuromuskulaere Erkrankungen und Basalganglienerkrankungen (z. B. Morbus Parkinson). Zudem sind in der Literatur Berichte ueber strahleninduzierte Kamptokormie und DHS vorhanden. In einer systematischen Recherche in PubMed mit den Begriffen ''camptocormia'', ''dropped head syndrome'', ''radiation-induced movment disorder'', ''radiation-induced myopathy'', ''radiation-induced neuropathy'' wurden alle Publikationen zu diesen Themen erfasst und mit dem Ziel analysiert, strahleninduzierte Bewegungsstoerungen und die verwendeten Bestrahlungstechniken naeher zu charakterisieren. Zudem beschreiben wir den Fall eines Patienten, der 23 Jahre nach Bestrahlung eines Non-Hodgkin-Lymphoms des Abdomens eine erhebliche, schmerzhafte Kamptokormie entwickelte. Es wurden 9 Fallberichte zu radiogenem DHS (n = 45 Patienten) und 3 Fallberichte (n = 3 Patienten) zu radiogener Kamptokormie gefunden. Die ueberwiegende Anzahl der beschriebenen Faelle (40/45 Patienten) trat > 15 Jahre nach Bestrahlung auf, ueberwiegend von Hodgkin-Lymphomen mit ausgedehnten Bestrahlungsfeldern. Der Einsatz ausgedehnter Bestrahlungsfelder unter multisegmentalem Einschluss von Wirbelsaeule und paraspinaler Muskulatur kann zum Auftreten von DHS nach Bestrahlung der Nackenmuskulatur und zu Kamptokormie nach Bestrahlung der Rueckenstrecker

    . (orig.) [German] Das Hodgkin-Lymphom (HL) ist eine Erkrankung mit hohen Heilungsraten. Die Verringerung von Spaetkomplikationen und Zweittumoren wird daher immer wichtiger. Zielvolumenkonzepte der Strahlentherapie (RT) veraendern sich gegenwaertig und Strahlentherapietechniken entwickeln sich sehr schnell weiter. Diese Uebersichtsarbeit stellt dar, inwiefern die Zielvolumenreduktion hin zum Involved-node(IN)-Konzept und hochentwickelte Strahlentherapietechniken wie die intensitaetsmodulierte Strahlentherapie (IMRT) und Protonentherapie, im Vergleich zu Involved-field(IF)-Konzept und 3-D-konformaler Strahlentherapie (3D-RT), die Belastung von Risikoorganen (OAR) mit hohen Dosen reduzieren koennen und welche Fragen in diesem Kontext noch geklaert werden muessen. Obwohl kein Vergleich aller verfuegbaren Techniken auf identischen Patientendatensaetzen existiert, entsteht folgendes Bild: Fortgeschrittene Dosisberechnungsalgorithmen (z. B. convolution-superposition/Monte Carlo) sollten im Rahmen der Behandlung des mediastinalen HL zur Anwendung kommen. INRT reduziert unter allen Bedingungen die behandelten Volumina im Vergleich zur IFRT, wobei die Hoehe des Vorteils von der jeweiligen INRT-Definition abhaengt. Die Anzahl der Patienten, die deutlich von hochkonformalen Techniken wie IMRT gegenueber der 3D-RT hinsichtlich der OAR-Belastung profitiert, ist bei INRT geringer. Die Konsequenz groesserer Volumina, die bei modernen Techniken mit eher isotroper Strahlanordnung mit niedrigen Dosen belastet werden, ist unklar. Die Art der verwendeten IMRT-Technik (statisch/Rotation) ist von geringer Relevanz. Alle fortgeschrittenen Photonentechniken resultieren in den gleichen Vorteilen und Nachteilen. Daher muss nur die Modulationstiefe abhaengig von den individuellen Behandlungszielen gewaehlt werden. Die Bestrahlung in tiefer Inspiration wird gegenwaertig evaluiert. Protonentherapie kann theoretisch bei hervorragender Hochdosiskonformalitaet die applizierte Integraldosis reduzieren

    Full Text Available FUNDAMENTO: O potencial de renovação e proliferação dos cardiomiócitos, in vivo, é pequeno, e por isso, o músculo cardíaco apresenta limitada capacidade de repor células perdidas. Na tentativa de minimizar os danos oriundos de lesões hipóxico-isquêmicas e daquelas que acometem o sistema de condução do coração, a terapia celular com células-tronco mesenquimais (MSC vem sendo utilizada, inclusive com cardiomiócitos diferenciados a partir de MSC. OBJETIVO: O presente trabalho comparou três protocolos distintos de indução de diferenciação objetivando a sugestão de um método viável para a diferenciação de maior número de células funcionais que expressem fenótipo cardiomiogênico. MÉTODOS: Culturas de MSC obtidas de tecido adiposo de ratos jovens da linhagem Lewis transgênicos para proteína verde fluorescente (GFP foram submetidos a três diferentes meios de diferenciação cardiogênica: Planat-Bérnard, 5-azacitidina e meio Planat-Bérnard + 5-azacitidina e observadas quanto a expressão de marcadores celulares cardíacos. RESULTADOS: Nos três protolocos utilizados observou-se formação da proteína alfa-actinina sarcomérica no citoesqueleto das células submetidas à diferenciação, expressão de conexina 43 na membrana nuclear e citoplasmática e formação de gap junctions, necessárias para a propagação do impulso elétrico no miocárdio, contudo, em nenhum protocolo foi observada contração espontânea das células submetidas à diferenciação cardiogênica.     Micro aggregates (10 - 20 nm) of human serum albumin I{sup 131} are shown to be superior to other agents for liver-spleen photoscanning. Radiation exposure to these organs is minimized and the test agent can be prepared with relative ease and simplicity. Also, the heart, stomach and salivary glands may be clearly visualized with the same material. The techniques and physiological basis for performing and interpreting scans of these organs are described. Their diagnostic value is illustrated. The physiological basis for using macro albumin aggregates (1 - 10 {mu}m) for visualization of the lungs is presented, along with typical scans in dogs. Toxicity studies and related pathophysiological investigations in animals demonstrate a wide margin of safety for using the large-particle test agents in man. Preliminary experiments in dogs indicate the feasibility of performing lung scans following the inhalation of an aerosol of the same material. This technique is simple, painless and entirely safe. Photoscans are presented from patients with a wide variety of pulmonary disorders.     des Hodgkin-Lymphoms (HL) reduziert. Der aktuelle Standard in der Behandlung besteht fuer die meisten HL-Patienten im Fruehstadium aus einer kombinierten Therapie. Ziel dieser Studie war es, die Stellen der Rezidive in Abhaengigkeit von den RT-Zielvolumina zu analysieren. Zwischen 1987 und 2011 wurden 427 HL-Patienten in Stadium I/II in unserer Institution mit einer RT mit oder ohne Chemotherapie behandelt. Die retrospektive Untersuchung umfasste 67 Patienten mit einem Rezidiv. Die meisten Patienten wiesen einen nodulaer-sklerosierenden Subtyp (86 %) und/oder ein Stadium II (75,9 %) auf. Von den Patienten hatten 21 % ein ''Bulky disease'' und 56 % eine unguenstige Prognose gemaess der Definition der EORTC (European Organization for Research and Treatment of Cancer) oder der LYSA (The Lymphoma Study Association). Eine kombinierte Therapie wurde 91 % der Patienten verabreicht. Bei allen Patienten wurde eine RT mit einer Dosis zwischen 20 und 45 Gy (mean =34 ± 5,3 Gy) durchgefuehrt. Eine Involved-Field-RT (IF-RT) kam bei 59 % der Patienten zum Einsatz. Die durchschnittliche Zeit zwischen der Diagnostik und dem Auftreten der Rezidive betrug 4,2 Jahre (Spanne 0,3-24,5 Jahre). Die Rezidive traten bei 53 % der Patienten ausserhalb des RT-Zielvolumens auf und waren bei Patienten mit einer guenstigen Prognose haeufiger. Rezidive innerhalb des RT-Zielvolumens waren mit einem ''Bulky disease'' assoziiert. Rezidive traten bei Patienten mit guenstiger Prognose spaeter auf als bei Patienten mit unguenstiger Prognose (3,5 vs. 2,9 Jahre; p = 0,5). In multivariater Analyse waren sowohl die Dosis als auch die Groesse der RT-Volumina nicht praediktiv fuer ein Rezidiv im Bestrahlungsfeld (jeweils p = 0,25 und p = 0,8), sondern nur das Bestehen eines ''Bulky disease'' (p = 0,018). [German] Die konsolidierende Radiotherapie (RT) in Kombination mit einer Chemoimmuntherapie stellt eine hocheffiziente Therapiemethode in der Behandlung des diffusen grosszelligen B-Zell-Lymphoms (DLBCL) dar. Die vorliegende retrospektive Analyse evaluiert die Effektivitaet und Sicherheit des Volumen- und Dosiskonzepts der ''Involved-site''-Radiotherapie (ISRT). Wir identifizierten 60 Patienten mit histologisch gesichertem DLBCL im Stadium I-IV, die zwischen Januar 2005 und Dezember 2015 mit einer Chemoimmuntherapie und konsolidierender ISRT behandelt wurden. Progressionsfreies (PFS) und Gesamtueberleben (OS) wurden mittels Kaplan-Meier-Methode dargestellt und univariate Analysen mittels Log-rank-Test und Mann-Whitney-U-Test erhoben. Nach einer initialen Chemoimmuntherapie (ueberwiegend R-CHOP; Rituximab, Cyclophosphamid, Doxorubicin, Vincristin und Prednisolon) erzielten 19 Patienten (36 %) eine komplette (CR) und 34 (64 %) eine partielle Remission (PR) oder weniger. Ausgeschlossen wurden 7 Patienten (12 %) mit einem Progress nach Chemoimmuntherapie. Alle Patienten erhielten eine ISRT mit 40 Gy Gesamtdosis. Nach einer medianen Nachbeobachtungszeit von 44 Monaten waren 79 % der Patienten erkrankungsfrei, waehrend 21 % ein Rezidiv oder einen systemischen Progress erlitten oder verstarben. Alle Patienten mit CR nach Chemoimmuntherapie blieben nach konsolidierender ISRT in CR. Bei 2 Patienten mit PR nach Chemoimmuntherapie trat ein Rezidiv an Stelle der initialen Laesion innerhalb des IS-Planungszielvolumens (PTV) auf. Feldrandrezidive wurden nicht beobachtet. Con el centelleografo del Oak Ridge National Laboratory (colimador de oro-wolframio enfocado, de 61 canales) los autores han podido delinear claramente la medula inyectando oro-198 coloidal por vfa endovenosa, ya que se sabe que la funcion reticuloendotelial de la medula esta relacionada con zonas de hematopoyesis. Comme le thiocyanate pentavalent se dissout dans une emulsion de corps gras, {sup 99m}TcO{sub 4}{sup -} se localise en grande partie dans le foie; la aussi, le taux de comptage eleve permet des explorations rapides avec une meilleure resolution.
