The study also underscores the immense investment — $150 billion — that states and localities have already made in educating these young Americans. It argues that over time and with a path to citizenship the return on this investment will increase by virtually every indicia of integration — education levels, employment rates, self-employment numbers, US family members, and English language proficiency.

  18. The influence of loan repayment on rural healthcare provider recruitment and retention in Colorado.

    Science.gov (United States)

Renner, Daniel M; Westfall, John M; Wilroy, Lou Ann; Ginde, Adit A

    2010-01-01

There is an ongoing shortage of rural healthcare providers relative to urban healthcare providers worldwide. Many strategies have been implemented to increase the distribution of rural healthcare providers, and financial incentives such as loan repayment programs have become popular means to both recruit and retain healthcare providers in rural communities. Studies detailing the effects of such programs on rural provider recruitment and retention are limited. The objective of this study was to assess the influence of loan repayment and other factors on the recruitment and retention of healthcare providers in rural Colorado, USA, and to compare the motivations and attitudes of these rural providers with their urban counterparts. A survey was sent to 122 healthcare providers who had participated in one of three loan repayment programs in Colorado between the years of 1992 and 2007: the Colorado Health Professional Loan Repayment Program; the Colorado Rural Outreach Program; and the Dental Loan Repayment Program of Colorado. Differentiation between rural and urban communities was accomplished by using the Rural Urban Commuting Area Codes developed by the University of Washington's Rural Health Research Center and Economic Research Service. Statistical analysis was performed using STATA from StataCorp. Of the 93 respondents included in the study, 57 worked in rural communities and 36 worked in urban communities during their programs. Of the rural participants, 74% were already working in or intending to work in an eligible community when they were made aware of the loan repayment program. Of those planning to work in a rural community regardless of any loan repayment option, 42% reported that the loan repayment program had an important influence on the specific community in which they chose to practice. Of the rural participants already working in a rural community, 38% reported loan repayment as being an important factor in their retention. The most important factors

  19. Racial and Ethnic Disparities in Meeting MTM Eligibility Criteria Based on Star Ratings Compared with the Medicare Modernization Act.

    2018-02-01

Previous research found racial and ethnic disparities in meeting medication therapy management (MTM) eligibility criteria implemented by the Centers for Medicare & Medicaid Services (CMS) in accordance with the Medicare Modernization Act (MMA). To examine whether alternative MTM eligibility criteria based on the CMS Part D star ratings quality evaluation system can reduce racial and ethnic disparities. This study analyzed the Beneficiary Summary File and claims files for Medicare beneficiaries linked to the Area Health Resource File. Three million Medicare beneficiaries with continuous Parts A, B, and D enrollment in 2012-2013 were included. Proposed star ratings criteria included 9 existing medication safety and adherence measures developed mostly by the Pharmacy Quality Alliance. Logistic regression and the Blinder-Oaxaca approach were used to test disparities in meeting MMA and star ratings eligibility criteria across racial and ethnic groups. Multinomial logistic regression was used to examine whether there was a disparity reduction by comparing individuals who were MTM-eligible under MMA but not under star ratings criteria and those who were MTM-eligible under star ratings criteria but not under the MMA. Concerning MMA-based MTM criteria, main and sensitivity analyses were performed to represent the entire range of the MMA eligibility thresholds reported by plans in 2009, 2013, and proposed by CMS in 2015. Regarding star ratings criteria, meeting any 1 of the 9 measures was examined as the main analysis, and various measure combinations were examined as the sensitivity analyses. In the main analysis, adjusted odds ratios for non-Hispanic blacks (backs) and Hispanics to non-Hispanic whites (whites) were 1.394 (95% CI = 1.375-1.414) and 1.197 (95% CI = 1.176-1.218), respectively, under star ratings. Blacks were 39.4% and Hispanics were 19.7% more likely to be MTM-eligible than whites. Blacks and Hispanics were less likely to be MTM-eligible than whites in some

  20. Credit reporting, relationship banking, and loan repayment

Directory of Open Access Journals (Sweden)

    2015-06-01

Full Text Available This paper presents an empirical investigation to determine factors influencing on loan repayment in one of Iranian banks named Sepah Bank over the period 2012-2013. The study selects a sample of 290 bank’s customers who received loans and, using logistic regression technique, tries to find whether or not qualitative as well as quantitative characteristics of loan receivers influence on repayment of loans. The results indicate that history of outstanding debt as well as customers’ past experiences with banks had meaningful relationships with having bad credit and non-payment of loans. In our survey, having a bad credit in the past had positive relationship with non-payment of loans but long-term customers had negative relationship with non-payment of loans. In addition, working capital turnover ratio, cash ratio, total liabilities, current assets and loan value had significant impact on non-repayment of the loan facilities.

  16. Medicare home health care patient case-mix before and after the Balanced Budget Act of 1997: effect on dual eligible beneficiaries.

    Science.gov (United States)

Shih, Huai-Che; Temkin-Greener, Helena; Votava, Kathryn; Friedman, Bruce

    2014-01-01

The Balanced Budget Act (BBA) of 1997 changed the payment system for Medicare home health care (HHC) from cost-based to prospective reimbursement. We used Medical Expenditure Panel Survey data to assess the impact of the BBA on Medicare HHC patient case-mix measured by the Centers for Medicare and Medicaid Services Hierarchical Condition Categories (CMS-HCC) model. There was a significant increase in Medicare HHC patient case-mix between the pre-BBA and Prospective Payment System (PPS) periods. The increase in the standardized-predicted risk score from the Interim Payment System period to PPS was nearly 4 times greater for the dual eligibles (Medicare-Medicaid) than for the Medicare-only population. This significantly greater rise in the HHC resources required by dual eligibles as compared to nonduals could be due to a shift in HHC payers from Medicare only to Medicaid rather than be an actual increase in case-mix per se.

Full Text Available Opinions of the regional accounting chambers on the possibility of repayment of the credit, loan or redemption of the securities by local government units have some specific features compared with the other opinions concerning the financial management of the local government. Firstly, they are only issued at the request of the unit that they concerned. Secondly, for the reason they relate to public debt issues, to some extent their content may be similar or “partial reproduction” of the standpoint of the regional chamber of accounting within already issued opinions referring to acts on budgetary planning process in local government units. The purpose of this article is to assess the legitimacy of issuing such opinions and to identify the relationship between the ordinances (resolutions on the possibility of repayment of the credit, loan or redemption of the securities and provisions of local government budget.

  19. ANALYSIS OF MODELS OF EARLY DEBT REPAYMENT IN THE Generalized CREDIT TRANSACTIONS

Directory of Open Access Journals (Sweden)

    2016-01-01

Full Text Available This paper analyzes the patterns of early repayment in multi-period credit transactions. Considered one of the most common ways of conversion of unpaid interest for early repayment, so-called 78 rule. The relationship of this rule with the linear approximation of the exact value; redeemable debt is determined. The analysis of the maximum excess payment of interest on 78 rule. It has been shown how interest payment on 78 rule depended on the time of early repayment. Early repayment of debt is an agreement under which the borrower pays to the lender amount of money equal to the current balance (as of loan account. Then further regular payments cease and the contract terminates. However, the amount of outstanding debt is determined by the structure of prescription charges. So in the uniform schemes of repayment of consumer credit each payment contains the same part of principal amounts and the total interest. In case of early repayment the Bank loses a significant fraction of the expected interest payments. Therefore, in practice, often used so-called accelerated schemes of interest payments. One of them is 78 rule. Use the 78 rule is simple and straightforward. The name of the rule is due to the fact that the sum of the numbers 12 monthly payments is 78. In the schemes of consumer loan with a term of one year interest payment for the current month is equal to m/78 of the total amount of interest payments, where m is the number of remaining payments. The rule name is stored and in the more general case with an arbitrary number of payments. In general interest payment is determined by the relative weight of the total amount of interest in each payment. In uniform schemes it is constant. In accelerated with a particular speed decreases. Therefore, additional cash expenses by the 78 rule may be considered as additional penalties for early repayment of the debt. It this article is shown how this penalty depends on time before maturity. It is shown that

  3. Prioritization of Factors Affecting the Delay or Inability to Repay Bank Facilities to Farmers

Directory of Open Access Journals (Sweden)

    B. Maadanian

    2016-03-01

Full Text Available Introduction: The role of credits in agricultural development is very important, especially after implementing land reform and converting subjects to a large class of small owners, the demand has been intensified. Seasonality of agricultural productions usually creates temporary vacuum among farmers payments and receipts, thus farmers need to save their previous incomes or seek financial help out of the sector in order to pay current expenses and investments in agriculture sector. Due to farmers low income, the saving possibility is low and therefore farmers are not in a situation that they can invest in agriculture sector from their savings or to purchase required inputs Materials and Methods: The aim of this study was to prioritize the factors affecting the delay and lack in repaying loans, therefore at first it should identify the payment factors. This means that one should find what factors are affecting the lack or delay in repayment of loans granted to farmers. So the factors affecting the delay or failure to repay the loans were identified by using the Delphi method and then prioritizing the factors will be discussed with regard to experts perspectives by using the analytical network process model. Analytical network process (ANP is one of the most efficient techniques for decision making with multiple criteria that it was proposed by Thomas Almaty for the first time in 1982 and as the developed form of AHP method. In cases where lower levels affect the upper levels or elements in a same level are not independent of each other, AHP method cannot be used. ANP technique is a more general form of AHP, but it does not require the hierarchical structure and therefore it show more complex relationships between different levels of decision in network form and it considers the interactions and feedbacks between criteria and alternatives. In fact the main objective of this process is to determine the overall impact of all factors in the face

Institute of Scientific and Technical Information of China (English)

    2011-01-01

To find out graduates’ repayment awareness of student loan,we conducted a questionnaire survey for those graduates who applied for student loan in Zhongkai University of Agriculture and Engineering. The survey has following results.First, the national student loan plays an important role in assisting poor students in finishing their study.Second, graduates value social function of personal credit.Third, trustworthiness education activities carried out by colleges and universities are effective.Fourth, economic income is a major factor of graduates repaying capital with interest.Fifth, bank’s student loan management system is not perfect.Sixth, the national student loan system remains to be improved. In line with these results,we put forward five countermeasures and suggestions:strengthen the trustworthiness education of students;standardize the credit investigation management of students’ personal credit;establish student information management mechanism;standardize banks’ payment reminder administration behavior;and perfect national student loan system.

Full Text Available The potency of Micro and Small Enterprises (MSEs in Indonesia which is great, is not directly proportional to the ease of getting capital from formal financial institutions such as banks because it is not bankable. Meanwhile, microfinance institutions (MFIs that provide financing to the MSEs are currently thriving. This condition exposes the existence of factors that cause the MFIs continuesly providing financing to MSEs which are not bankable. This research aims to analyze the effect of social capital toward the repayment rate of Islamic MFIs’ customers. The methods used in this research was the logistic regression. The results showed variable relationship with BMT employees, relations with other customers, and Islamic recitation had positive and significant effect toward the repayment rate of the customer. While recommendations, membership status, and travel time from home to the BMT had negative and significant effect.  Variable outside the social capital indicator which is income rate had no significant effect.DOI: 10.15408/aiq.v8i2.2631

  11. States' experiences with loan repayment programs for health care professionals in a time of state budget cuts and NHSC expansion.

    Science.gov (United States)

Pathman, Donald E; Morgan, Jennifer Craft; Konrad, Thomas R; Goldberg, Lynda

    2012-01-01

The landscape of education loan repayment programs for health care professionals has been turbulent in recent years, with doubling of the funding for the National Health Service Corps (NHSC) and cuts in funding for some states' programs. We sought to understand how this turbulence is being felt within the state offices involved in recruiting clinicians to rural and urban underserved communities. We conducted key informant telephone interviews with staff of state offices of rural health, primary care organizations, and/or related organizations within 28 diverse states to answer questions about perceived changes and interplay among solely state-funded loan repayment programs, joint state-federal programs, and the NHSC federal program. Interviews were transcribed, formally analyzed, and key issues summarized. Informants reported that solely state-funded and joint state-federal loan repayment programs are greatly valued for their ability to target a state's particular needs and to complement the NHSC federal program. However, budgets for state programs have been threatened, reduced, or eliminated entirely in many cases. All informants positively perceived the NHSC's recent growth and changes, which they feel are helping fill important workforce needs for their states. Nevertheless, the much larger NHSC federal program now competes with some states' programs for clinicians and service sites; states' programs are pushed to adjust their operations to maintain a unique "niche". States' key recruiters lament reductions in funding for states' loan repayment programs, and welcome the NHSC's recent growth and changes. Better coordination is needed to minimize competition and maximize complementarity between state and federal programs. © 2012 National Rural Health Association.

    2016-04-01

Full Text Available This paper explores the influence of macroeconomic indicators, namely GDP growth, the Consumer Price Index and the unemployment rate on the quality of loan repayments by households in the banking market of the Federation of Bosnia and Herzegovina. Potential influence is observed over a period of fourteen years at the level of nonperforming household loans using regression analysis. The authors aim to determine whether macroeconomic forces actually influence loan repayment, and if so how and what can be done by banks to utilize this information in order to reduce future credit losses, and by the government to maintain the stability of the banking sector.

  5. A Comparison of the J-1 Visa Waiver and Loan Repayment Programs in the Recruitment and Retention of Physicians in Rural Nebraska.

    Science.gov (United States)

Opoku, Samuel T; Apenteng, Bettye A; Lin, Ge; Chen, Li-Wu; Palm, David; Rauner, Thomas

    2015-01-01

There is a dearth of literature evaluating the effectiveness of programs aimed at recruiting and retaining physicians in rural Nebraska. Taking advantage of the Nebraska Health Professional Tracking System, this study attempts to comparatively assess the effectiveness of the J-1 visa waiver and state loan repayment programs in the recruitment and retention of physicians in rural Nebraska. A mixed methods approach was used. We tracked 240 physicians who enrolled in the J-1 visa waiver and state loan repayment programs between 1996 and 2012 until 2013. In addition, key informant interviews were conducted to obtain perspectives on the recruitment and retention of physicians in rural Nebraska through the 2 programs. Results from multilevel survival regression analysis indicated that physicians enrolled in the J-1 visa waiver program were more likely to leave rural Nebraska when compared with those enrolled in the state loan repayment program. Participants in the qualitative study, however, cautioned against declaring one program as superior over the other, given that the 2 programs addressed different needs for different communities. In addition, results suggested that fostering the integration of physicians and their families into rural communities might be a way of enhancing retention, regardless of program. The findings from this study highlight the complexity of recruitment and retention issues in rural Nebraska and suggest the need for more holistic and family-centered approaches to addressing these issues. © 2015 National Rural Health Association.

  9. Lower rates, better service, accelerated debt repayment: how best to sell a minority interest in Hydro One

    International Nuclear Information System (INIS)

    2002-01-01

The sale of a minority stake in the provincially-owned, integrated electricity transmission and distribution company Hydro One is being contemplated by the Ontario government. Several options are open to the government to complete this sale, such as an income trust, an Initial Public Offering (IPO), straight sale of 49.9 per cent or less, and the separation and sale of the distribution operations. Some issues must be considered before proceeding with the divestiture: service quality and the current structure of the distribution sector in Ontario, the distribution and transmission rates, fostering competition in distribution, regulatory costs, tax leakage/stranded debt repayment, maximization of value to the province, public/stakeholder acceptance, foreign versus domestic ownership, accountability to consumers, and the policies/requirements of interconnected markets. The aim in the divestiture is to ensure customers in Ontario benefit from lower distribution rates, higher quality services, enhanced local accountability, a more efficient electricity industry, and accelerated stranded debt payoff. As a result, the Electricity Distributors Association is proposing that the government separate Hydro One's transmission and distribution. The rationale for the proposal was discussed in the paper, stressing the importance of making the decision now

  3. Eligibility in the electricity and gas retail markets in Spain

    International Nuclear Information System (INIS)

    Marti, F.

    2003-01-01

The true liberalization of the electricity and gas retail markets in Spain was initiated in 1997 and 1998 respectively with the approval of the Electricity Act and the Hydrocarbons Act that set up the regulatory framework for both sectors. The initial eligibility calendars established were speeded up by later legal dispositions which set the full eligibility in January 2003. Therefore in January the number of eligible consumers increased from 65,000 electricity consumers to 22 million and from around 5,000 gas consumers to 4.5 million. In Spain there has been however a coexistence of a liberalized market and a regulated market. While in the liberalized market the consumer will pay a regulated Third Party Access (TPA) tariff for the use of the network and a free price for the energy, in the regulated market the consumer will pay all-inclusive tariffs which are established by the Government in the case of the electricity tariffs and by the Ministry of Economy in the case of the gas tariffs. Today any electricity and gas consumer can choose whether he wants to be supplied through the regulated market or through the liberalized one. Only the electricity HV (High Voltage Supply) all-inclusive tariffs are to disappear in 2007. No date has been set for the others. Also important is to highlight the increase in the consumption of both energy sources. The electricity demand has increased about 38 percent from 1997 (155 TWh/year) to 2002 (214 TWh/year). The gas sector, on the other hand, has increased at a higher rate. From 150 TWh/year in January 1999 to almost 270 TWh/year in July 2003, an 80 percent increase in four and a half years.(author)

  4. DETERMINATION OF THE TIME LIMITS OF ELIGIBILITY IN A STATE OF EXTREME NECESSITY

Directory of Open Access Journals (Sweden)

    Vyacheslav Victorovich Naumov

    2015-12-01

Full Text Available Up to now the limits of eligibility of extreme necessity were not the subject of independent research. The problems of definition of the time limits of p eligibility are considered in this article based on the scientific and monographic literature analysis. Relevance of the considered matter is defined by the fact that the correct establishment of time limits of eligibility of act is of great importance in the law enforcement activity in order to establish the non-delinquency of act in state of extreme necessity. The purpose of this research is the analysis of definition of time limits of extreme necessity eligibility and drawing proposals on this basis for improvement of legislative regulations and existing laws enforcement efficiency with regards to extreme necessity. Research methods: dialectic, formal-logical, system. The author empha-sizes the eligibility limits of admissible harm and the temporary limits. When determining the time limits the author emphasizes the following criteria: the value of danger, intensity of actions, and also identifies an "initial" and "final" moments in the defense of legally protected interests. The improvement of the existing legisla-tion was proposed based on the carried-out analysis.

  5. Investment and repayment in a trust game after ventromedial prefrontal damage

Directory of Open Access Journals (Sweden)

    Giovanna eMoretto

    2013-09-01

Full Text Available Although trust and reciprocity are ubiquitous in social exchange, their neurobiological substrate remains largely unknown. Here, we investigated the effect of damage to the ventromedial prefrontal cortex (vmPFC — a brain region critical for valuing social information — on individuals' decisions in a trust game and in a risk game. In the trust game, one player, the investor, is endowed with a sum of money, which she can keep or invest. The amount she decides to invest is tripled and sent to the other player, the trustee, who then decides what fraction to return to the investor. In separate runs, ten patients with focal bilateral damage to the vmPFC and control participants made decision while playing in the role of either investor or trustee with different anonymous counterparts in each run. A risk game was also included in which the investor faced exactly the same decisions as in the trust Game, but a random device (i.e., a computer, not another player, determined the final payoffs. Results showed that vmPFC patients’ investments were not modulated by the type of opponent player (e.g., human vs. computer present in the environment. Thus, vmPFC patients showed comparable risk-taking preferences both in social (trust game and nonsocial (risk game contexts. In stark contrast, control participants were less willing to take risk and invest when they believed that they were interacting with people than a computer. Furthermore, when acted as trustee, vmPFC patients made lower back transfers toward investors, thereby showing less reciprocity behavior. Taken together, these results indicate that social valuation and emotion subserved by vmPFC have a critical role in trusting and reciprocity decisions. The present findings support the hypothesis that vmPFC damage may impair affective systems specifically designed for mediating social transaction with other individuals.

  6. Medicaid and Children's Health Insurance Programs: essential health benefits in alternative benefit plans, eligibility notices, fair hearing and appeal processes, and premiums and cost sharing; exchanges: eligibility and enrollment. Final rule.

    Science.gov (United States)

    2013-07-15

This final rule implements provisions of the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010 (collectively referred to as the Affordable Care Act. This final rule finalizes new Medicaid eligibility provisions; finalizes changes related to electronic Medicaid and the Children's Health Insurance Program (CHIP) eligibility notices and delegation of appeals; modernizes and streamlines existing Medicaid eligibility rules; revises CHIP rules relating to the substitution of coverage to improve the coordination of CHIP coverage with other coverage; and amends requirements for benchmark and benchmark-equivalent benefit packages consistent with sections 1937 of the Social Security Act (which we refer to as ``alternative benefit plans'') to ensure that these benefit packages include essential health benefits and meet certain other minimum standards. This rule also implements specific provisions including those related to authorized representatives, notices, and verification of eligibility for qualifying coverage in an eligible employer-sponsored plan for Affordable Insurance Exchanges. This rule also updates and simplifies the complex Medicaid premium and cost sharing requirements, to promote the most effective use of services, and to assist states in identifying cost sharing flexibilities. It includes transition policies for 2014 as applicable.

Acupuncture is covered under the Accident Compensation (Liability to Pay or Contribute to Cost of Treatment) Regulations 2003, and is therefore eligible for Accident Compensation Corporation payments for the treatment of personal injuries. This study searched New Zealand acupuncturists' websites for therapeutic claims that may breach Section 58(1)(a) of the Medicines Act. A search of acupuncturists' websites shows that many claim to be able to treat a wide range of conditions, despite a lack of evidence showing the efficacy of acupuncture in the treatment of those conditions. Practitioners and owners of websites likely to be in breach of the Medicines Act include many committee members from acupuncture's professional bodies.

The Individuals with Disabilities in Education Improvement Act of 2004 mandates the special education process: identification, full and individual evaluation, eligibility determination, and development of the individual education program (IEP), including special education placement. Part 1 focused on the importance of the school nurse's role in student identification, response to intervention, and the full and individual evaluation. Part 2 highlights the school nurse's vital and unique contribution to the subsequent special education steps of eligibility determination, IEP development, and special education services placement and minutes.

  18. An empirical analysis of the cost of rearing dairy heifers from birth to first calving and the time taken to repay these costs.

    2017-08-01

Rearing quality dairy heifers is essential to maintain herds by replacing culled cows. Information on the key factors influencing the cost of rearing under different management systems is, however, limited and many farmers are unaware of their true costs. This study determined the cost of rearing heifers from birth to first calving in Great Britain including the cost of mortality, investigated the main factors influencing these costs across differing farming systems and estimated how long it took heifers to repay the cost of rearing on individual farms. Primary data on heifer management from birth to calving was collected through a survey of 101 dairy farms during 2013. Univariate followed by multivariable linear regression was used to analyse the influence of farm factors and key rearing events on costs. An Excel spreadsheet model was developed to determine the time it took for heifers to repay the rearing cost. The mean±SD ages at weaning, conception and calving were 62±13, 509±60 and 784±60 days. The mean total cost of rearing was £1819±387/heifer with a mean daily cost of £2.31±0.41. This included the opportunity cost of the heifer and the mean cost of mortality, which ranged from £103.49 to £146.19/surviving heifer. The multivariable model predicted an increase in mean cost of rearing of £2.87 for each extra day of age at first calving and a decrease in mean cost of £6.06 for each percentile increase in time spent at grass. The model also predicted a decrease in the mean cost of rearing in autumn and spring calving herds of £273.20 and £288.56, respectively, compared with that in all-year-round calving herds. Farms with herd sizes⩾100 had lower mean costs of between £301.75 and £407.83 compared with farms with economic analysis suggest that management decisions on key reproduction events and grazing policy significantly influence the cost of rearing and the time it takes for heifers to start making a profit for the farm.

  20. THE ACCOUNTING OF THE PLEDGED ASSETS PASSED TO THE LICENSED BANKS FROM THE REPUBLIC OF MOLDOVA FOR THE REPAYMENT OF THE GRANTED LOAN

Directory of Open Access Journals (Sweden)

    2014-06-01

Full Text Available In banking practice the pledge plays an important role, it is often used to denote the pledge agreement, as well asthe real right that is the result of the pledge agreement conclusion and the good that is the object of the pledge. Thepledge is the only real security interest in the range of bank guarantees. Actually, each obligation can beguaranteed by the pledging. Assets diversification and trade needs have led to the development of a new pledgeform. Anyway, regardless of the number and content of the economic material, the licensed banks from the Republicof Moldova attach importance to the correctness of pledge accounting taking into account the deadline. Thus, thisresearch aims to present the accounting of the pledged assets passed to the licensed banks from the Republic ofMoldova for the repayment of the granted loan, taking into account the enforcement of the obligation secured by thepledge deadline, the right to possession of the pledged asset and not the least of its value. In the research there wasmainly used the method of monographic study applying elements of observation, selection, induction and deduction.

To examine the sources of family income dynamics leading to movement into and out of Medicaid expansion and subsidy eligibility under the Affordable Care Act. Survey of Income and Program Participation (SIPP): 1996, 2001, 2004, 2008 panels. Considering four broad subsidy eligibility categories for monthly Modified Adjusted Gross Income (MAGI) (400 percent FPL), I use duration analysis to examine determinants of movements between categories over the course of a year. Using detailed monthly data, I determine the members of tax-filing units and calculate an approximation of MAGI at the monthly level. The analysis sample is adults ages 22-64 years. Incomes are highly variable within a year, particularly at the lower end of the income distribution. Employment transitions, including transitions not involving a period of nonemployment, and family structure changes strongly predict sufficient income volatility to trigger a change in subsidy category. Income volatility arising from employment and family structure changes is likely to trigger changes in subsidy eligibility within the year, but the sources and effects of the volatility differ substantially depending on the individual's position in the income distribution. © Health Research and Educational Trust.

  4. Initial Readability Assessment of Clinical Trial Eligibility Criteria

    Science.gov (United States)

Kang, Tian; Elhadad, Noémie; Weng, Chunhua

    2015-01-01

Various search engines are available to clinical trial seekers. However, it remains unknown how comprehensible clinical trial eligibility criteria used for recruitment are to a lay audience. This study initially investigated this problem. Readability of eligibility criteria was assessed according to (i) shallow and lexical characteristics through the use of an established, generic readability metric; (ii) syntactic characteristics through natural language processing techniques; and (iii) health terminological characteristics through an automated comparison to technical and lay health texts. We further stratified clinical trials according to various study characteristics (e.g., source country or study type) to understand potential factors influencing readability. Mainly caused by frequent use of technical jargons, a college reading level was found to be necessary to understand eligibility criteria text, a level much higher than the average literacy level of the general American population. The use of technical jargons should be minimized to simplify eligibility criteria text. PMID:26958204

  20. The Radiation Protection Act

    International Nuclear Information System (INIS)

    Persson, L.

    1989-01-01

The new Radiation Protection Act (1988:220) entered into force in Sweden on July 1st, 1988. This book presents the Act as well as certain regulations connected to it. As previously, the main responsibility for public radiation protection will rest with one central radiation protection authority. According to the 1988 Act, the general obligations with regard to radiation protection will place a greater responsibility than in the past on persons carrying out activities involving radiation. Under the act, it is possible to adjust the licensing and supervisory procedures to the level of danger of the radiation source and the need for adequate competence, etc. The Act recognises standardised approval procedures combined with technical regulations for areas where the risks are well known. The Act contains several rules providing for more effective supervision. The supervising authority may in particular decide on the necessary regulations and prohibitions for each individual case. The possibilities of using penal provisions have been extended and a rule on the mandatory execution of orders has been introduced. The Ordinance on Radiation Protection (1988:293) designates the National Institute of Radiation Protection (SSI) as the central authority referred to in the Radiation Protection Act. The book also gives a historic review of radiation protection laws in Sweden, lists regulations issued by SSI and presents explanations of radiation effects and international norms in the area. 1. Canada's Clean Air Act

    International Nuclear Information System (INIS)

    2006-01-01

This paper provided an outline of Canada's Clean Air Act and examined some of the regulatory changes that will occur as a result of its implementation. The Act is being introduced to strengthen the legislative basis for taking action on reducing air pollution and GHGs, and will allow the government to regulate both indoor and outdoor air pollutants and GHGs. The Act will require the Ministers of the Environment and Health to establish national air quality objectives, as well as to monitor and report on their attainment. The Canadian Environmental Protection Act will be amended to enable the government to regulate the blending of fuels and their components. The Motor Vehicle Fuel Consumption Standards Act will also be amended to enhance the government's authority to regulate vehicle fuel efficiency. The Energy Efficiency Act will also be expanded to allow the government to set energy efficiency standards and labelling requirements for a wider range of consumer and commercial products. The Act will commit to short, medium and long-term industrial air pollution targets. Regulations will be proposed for emissions from industry; on-road and off-road vehicles and engines; and consumer and commercial products. It was concluded that the Government of Canada will continue to consult with provinces, territories, industries and Canadians to set and reach targets for the reduction of both indoor and outdoor air pollutants and GHG emissions. 6 figs

  2. Impact of the Medicare Catastrophic Coverage Act on nursing homes.

    1997-01-01

The Medicare Catastrophic Coverage Act (MCCA) of 1988 altered eligibility and coverage for skilled nursing facility (SNF) care and changed Medicaid eligibility rules for nursing-home residents. Detailed data on the residents of a for-profit nursing-home chain and Medicare claims for a 1 percent sample of beneficiaries were used to examine the impact of the MCCA on nursing homes. The case mix of nursing-home admissions was scrutinized, specifically for length of stay, discharge disposition, rate of hospitalization, and changes in payer source. Findings revealed that, although the proportion of Medicare-financed nursing-home care increased, as did the case-mix severity of residents during the MCCA period, there was no corollary reduction in hospital use by nursing-home residents.

    International Nuclear Information System (INIS)

    1983-01-01

Part II of This Act came into force on 1 September 1983 and is concerned with nuclear installations. Its main purpose is to amend the Nuclear Installations Act 1965 to give effect to the provisions of two Protocols amending the Paris Convention on nuclear third party liability and the Brussels Convention Supplementary to the Paris Convention respectively. The principal effect of these modifications is to increase the sums available to meet claims for nuclear damage. The United Kingdom is a Party to both Conventions and the provisions of the 1983 Act will enable it to ratify the Protocols. (NEA) [fr

  4. Medicaid Expansion And Marketplace Eligibility Both Increased Coverage, With Trade-Offs In Access, Affordability.

    Science.gov (United States)

Selden, Thomas M; Lipton, Brandy J; Decker, Sandra L

    2017-12-01

Affordable Care Act (ACA) provisions implemented in 2014 provide a valuable case study regarding the merits of using public versus subsidized private insurance to help low-income people obtain and finance health care. In particular, nonelderly adults with incomes of 100-138 percent of the federal poverty level gained Medicaid eligibility if they lived in states that implemented the ACA's Medicaid expansion, whereas those in nonexpansion states became eligible for subsidized Marketplace coverage. Using data for 2008-15 from the National Health Interview Survey, we found that as of 2015, adults with family incomes in this range had experienced large declines in uninsurance rates in both expansion and nonexpansion states (the adjusted declines were 22 percentage points and 18 percentage points, respectively). Adults in expansion and nonexpansion states also experienced similar increases in having a usual source of care and primary care visits, and similar reductions in delayed receipt of medical care due to cost. There were, however, important differences: Adults in expansion states experienced larger reductions in out-of-pocket spending but also faced greater difficulty accessing physician care relative to adults in nonexpansion states.

  5. The impact of CHIP premium increases on insurance outcomes among CHIP eligible children.

    Science.gov (United States)

    Nikolova, Silviya; Stearns, Sally

    2014-03-03

Within the United States, public insurance premiums are used both to discourage private health policy holders from dropping coverage and to reduce state budget costs. Prior research suggests that the odds of having private coverage and being uninsured increase with increases in public insurance premiums. The aim of this paper is to test effects of Children's Health Insurance Program (CHIP) premium increases on public insurance, private insurance, and uninsurance rates. The fact that families just below and above a state-specific income cut-off are likely very similar in terms of observable and unobservable characteristics except the premium contribution provides a natural experiment for estimating the effect of premium increases. Using 2003 Medical Expenditure Panel Survey (MEPS) merged with CHIP premiums, we compare health insurance outcomes for CHIP eligible children as of January 2003 in states with a two-tier premium structure using a cross-sectional regression discontinuity methodology. We use difference-in-differences analysis to compare longitudinal insurance outcomes by December 2003. Higher CHIP premiums are associated with higher likelihood of private insurance. Disenrollment from CHIP in response to premium increases over time does not increase the uninsurance rate. When faced with higher CHIP premiums, private health insurance may be a preferable alternative for CHIP eligible families with higher incomes. Therefore, competition in the insurance exchanges being formed under the Affordable Care Act could enhance choice.

The commentary is intended to contribute to protection of the population by a practice-oriented discussion and explanation of questions arising in connection with the Preventive Radiation Protection Act. Leaving aside discussions about abandonment of nuclear power, or criticism from any legal point of view, the commentary adopts the practical approach that accepts, and tries to help implementing, the act as it is. It is a guide for readers who are not experts in the law and gives a line of orientation by means of explanations and sometimes by citations from other acts (in footnotes). The commentary also presents the EURATOM Directive No. 3954/87 dated 22 December 1987, the EC Directive No. 3955/87 dated 22 December 1987, and the EC Directive No. 1983/88 dated 5 July 1988. International Nuclear Information System (INIS)

    1991-01-01

This Act will enter into force on 1 January 1992. The scope of the Act is extensive as, in addition to ionizing radiation, it will also apply to activities involving exposure to natural radiation and non-ionizing radiation. Its purpose is to prevent and restrict harmful effects to health resulting from radiation. The basic principles of the Act are that the practice involving radiation should be justified; radiation protection should be optimized; and radiation doses should be as low as reasonably achievable. Licensed organisations using radiation will be responsible for the safety of the activity involving exposure to radiation and for having available the appropriate expertise to this effect. The required so-called safety licence provides the regulatory control to ensure that radiation is used sensibly, that the equipment and shields are technically acceptable and the operating personnel is competent, and that the radioactive waste is dealt with appropriately. The Radiation Act will also apply to nuclear activities within the scope of the 1987 Nuclear Energy Act [fr

The Clean Air Act amendments alter the complex laws affecting atmospheric pollution and at the same time have broad implications for energy. Specifically, the Clean Air Act amendments for the first time deal with the environmental problem of acid deposition in a way that minimizes energy and economic impacts. By relying upon a market-based system of emission trading, a least cost solution will be used to reduce sulfur dioxide (SO 2 ) emissions by almost 40 percent. The emission trading system is the centerpiece of the Clean Air Act (CAA) amendments effort to resolve energy and environmental interactions in a manner that will maximize environmental solutions while minimizing energy impacts. This paper will explore how the present CAA amendments deal with the emission trading system and the likely impact of the emission trading system and the CAA amendments upon the electric power industry

Objectives of the study were to measure recruitment rates in clinical trials and to identify patients, physicians or trials characteristics associated with higher recruitment rates. Among patients who had a clinical trial available for their cancer, 83.5% (345/413) met the eligibility criteria to at least one clinical trial. At least one trial was proposed to 33.1% (113/341) of the eligible patients and 19.7% (68/345) were recruited. Overall recruitment was 16.5% (68/413). In multivariate analyses, trial proposal and enrollment were lower for elderly patients and higher in high cancer stages. Trials from pharmaceutical industry had higher recruitment rates and trials testing hormonal therapy enrolled more patients. Breast cancer patients' accrual to a clinical trial could be improved by trying to systematically identify all eligible patients and propose a trial to those eligible and to whom the treatment is planned to be equivalent to the standard arm of the trial. 4. Evidence of eligibility manipulation for conditional cash transfer programs

Directory of Open Access Journals (Sweden)

    Sergio Firpo

    2014-09-01

Full Text Available This paper assesses whether eligibility for conditional cash transfer programs has been manipulated, as well as the impact of this phenomenon on time allocation within households. To perform this analysis, we use data from the 2006 PNAD (Brazilian national household survey and investigate the eligibility manipulation for the Bolsa Família (Family Stipend program during this time period. The program assists families with a monthly per capita income of around R$120.00 (US$60.00. By applying the tests developed by McCrary (2008, we find suggestive evidence that individuals manipulate their income by voluntarily reducing their labor supply in order to become eligible to the program. Moreover, the reduction in labor supply is greater among women, especially single or divorced mothers. This evidence raises some concern about the unintended consequences related to the eligibility criteria utilized by Bolsa Família, as well as the program's impact on individuals living in extreme poverty.

  5. Crossing 138: two approaches to churn under the Affordable Care Act.

    Science.gov (United States)

    Ravel, Gabriel; DeSantis, J Angelo

    2014-01-01

A predicted side effect of the Medicaid expansion and state-based Exchanges under the Affordable Care Act is churn. Churn is the shifting into and out of eligibility for insurance affordability programs due to income changes. Because the line between Medicaid and Exchange eligibility is fine -138% of the federal poverty level -millions of Americans are expected to gain and lose eligibility. Frequently, this churning undermines continuity of care, raises costs, and frustrates those affected. This article explores two proposed programs to mitigate the effects of churn: the Basic Health Program and the Bridge Program. This article evaluates both programs' ability to mitigate the effects of churn, the likely side effects to states' implementing them, and legal and practical obstacles to their implementation. It concludes that the Bridge Program is the better approach.

  13. Dreams and acting out.

    Science.gov (United States)

    Grinberg, L

    1987-01-01

Dreams can be used as containers that free patients from increased tension. This may be the principal function of certain types of dreams, called "evacuative dreams." They are dreams used for getting rid of unbearable affects and unconscious fantasies, or as a safety valve for partial discharge of instinctual drives. These dreams are observed primarily in borderline and psychotic patients, but can also be seen in the regressive states of neurotic patients during weekends and other periods of separation. Such dreams have to be differentiated from "elaborative dreams," which have a working-through function and stand in an inverse relationship to acting out: the greater the production of elaborative dreams, the less the tendency to act out, and vice versa.
