Sonographic estimation of fetal weight (EFW) is important in the management of high-risk pregnancies. The possibility that increased maternal body mass index (BMI) adversely affects EFW assessments in twin pregnancies is controversial. The aim of this study was to investigate the effect of maternal BMI on the accuracy of EFW assessments in twin gestations prospectively recruited for the ESPRiT (Evaluation of Sonographic Predictors of Restricted growth in Twins) study.

Since, the ablation of organic materials by photo decomposition process using UV lasers have many different applications in fields such as optoelectronics, microelectronics and surgery, therefore an special attention is given to it. The time resolved information together with available mathematical models can provide a better understanding about ablation. As the ablation of materials begin a few nanoseconds after arrival of photons, thus it would be useful if one can study the ablation process in a time scale shorter than the laser pulse duration. In this article, the possible application of PVDF piezoelectric transducer to study the photoacoustic reaction of polymers against excimer lasers is described. Time resolved signals in two areas will be useful I- The thermoelastic region below threshold which gives some information about optical characteristics of target and also thermal relaxation time of excited states, II- Ablation time scale above threshold which provide us information regarding the nature of ablation mechanism. The experiments have shown that PET and polyimide polymers began to ablate within (4-6)ns of ArF pulse width at a fluence higher than 100 mJcm{sup -2}. This produced a short acoustic pulse of 20 ns with a maximum pressureof 10{sup 7} pa (100 atm). Ablation of PMMA using the same laser began within 8 ns. These results show that the polymers with high absorption coefficients are photochemically decomposed and those with low absorption coefficients such as PET at 308 nm, the ablation is brought about by thermal effects.

We sought to evaluate the association between placental histological abnormalities and birthweight discordance and growth restriction in twin pregnancies. We performed a multicenter, prospective study of twin pregnancies. Placentas were examined for evidence of infarction, retroplacental hemorrhage, chorangioma, subchorial fibrin, or abnormal villus maturation. Association of placental lesions with chorionicity, birthweight discordance, and growth restriction were assessed. In all, 668 twin pairs were studied, 21.1% monochorionic and 78.9% dichorionic. Histological abnormalities were more frequent in placentas of smaller twins of birthweight discordant pairs (P = .02) and in placentas of small for gestational age infants (P = .0001) when compared to controls. The association of placental abnormalities with both birthweight discordance and small for gestational age was significant for dichorionic twins (P = .01 and .0001, respectively). No such association was seen in monochorionic twins. In a large, prospective, multicenter study, we observed a strong relationship between abnormalities of placental histology and birthweight discordance and growth restriction in dichorionic, but not monochorionic, twin pregnancies. The purpose of this study was to evaluate the impact of noncentral placental cord insertion on birthweight discordance in twins. We performed a multicenter, prospective trial of twin pregnancies. Placental cord insertion was documented as central, marginal, or velamentous according to a defined protocol. Association of the placental cord insertion site with chorionicity, birthweight discordance, and growth restriction were assessed. Eight hundred sixteen twin pairs were evaluated; 165 pairs were monochorionic, and 651 pairs were dichorionic. Monochorionic twins had higher rates of marginal (P = .0068) and velamentous (P < .0001) placental cord insertion. Noncentral placental cord insertion was more frequent in smaller twins of discordant pairs than control pairs (29.8% vs 19.1%; P = .004). Velamentous placental cord insertion in monochorionic twins was associated significantly with birthweight discordance (odds ratio, 3.5; 95% confidence interval, 1.3-9.4) and growth restriction (odds ratio, 4; 95% confidence interval, 1.1-14.3). Noncentral placental cord insertion contributes to birthweight discordance in monochorionic twin pregnancies. Sonographic delineation of placental cord insertion may be of value in antenatal assessment of twin pregnancies. Copyright © 2011 Mosby, Inc. All rights reserved.

Full Text Available Infant malnutrition remains an important cause of death and disability, and Haiti has the highest prevalence in the Americas. Therefore, preventive strategies are needed. Our aims were (1 To assess the prevalence of malnutrition among young children seen at a health center in Haiti; (2 Examine adherence to infant feeding practices recommended by the World Health Organization (WHO and the association to nutritional status. This cross-sectional study recruited children from the Saint Espri Health Center in Port Au Prince in 2014. We recorded feeding practices, socio-demographic data, and anthropometric measurements (WHO-2006. We evaluated 278 infants and children younger than two years old, aged 8.08 ± 6.5 months, 53.2% female. 18.35% were underweight (weight/age <−2 SD; 13.31% stunted (length/age <−2 SD, and 13.67% had moderate or severe wasting (weight/length <−2 SD. Malnutrition was associated with male gender, older age, lower maternal education level, and greater numbers of siblings (Chi2, p < 0.05. Adherence to recommended breastfeeding practices was 11.8–97.9%, and to complementary feeding practices was 9.7–90.3%. Adherence was associated with a lower prevalence of malnutrition. Conclusion: Prevalence of infant and young child malnutrition in this population is high. Adherence to WHO-recommended feeding practices was associated with a better nutritional status.

  7. Dichorionic twin ultrasound surveillance: sonography every 4 weeks significantly underperforms sonography every 2 weeks: results of the Prospective Multicenter ESPRiT Study.

A 2-week ultrasound scanning schedule for monochorionic twins is endorsed widely. There is a lack of robust data to inform a schedule for the surveillance of dichorionic gestations. We aimed to determine how ultrasound scanning that is performed at 2- or 4-week intervals (or every 4 weeks before 32 weeks' gestation and every 2 weeks thereafter) may impact the prenatal detection of fetal growth restriction (FGR) and ultimately influence timing of delivery. In a consecutive cohort of 789 dichorionic twin pregnancies that were recruited prospectively for the multicenter Evaluation of Sonographic Predictors of Restricted Growth in Twins study, ultrasound determination of fetal growth and interrogation of umbilical and middle cerebral artery Doppler scans were performed every 2 weeks from 24 weeks' gestation until delivery. Complete delivery and perinatal outcome data were recorded for all pregnancies. Where delivery was prompted by FGR, abnormal umbilical artery Doppler examination or poor biophysical profile and in the absence of ruptured membranes, onset of labor, preeclampsia, or antepartum hemorrhage, the delivery was considered "ultrasound-indicated." For ultrasound-indicated deliveries, detection probabilities for FGR/abnormal umbilical artery Doppler scans/poor biophysical were determined according to the interval between examinations, by the suppression if alternate examination data. Among 789 dichorionic twin pregnancies, 66 pairs (8%) had an "ultrasound indicated" delivery. Detection of FGR was reduced from 88-69%, and detection of abnormal umbilical artery Doppler was reduced from 82-62% when a 4-week ultrasound schedule was simulated. Both of these reductions reached statistical significance. There was a nonsignificant trend toward a reduction in the recording of oligohydramnios with a 4-week interval between examinations. This study suggests that the ultrasound surveillance program of every 2 weeks that is recommended currently for monochorionic twins should be extended to dichorionic gestations. Given that most of these patients have respiratory and peripheral muscle weakness, dyspnea and functional exercise capacity may improve as a result of inspiratory muscle training (IMT. The present study is designed to investigate the benefits of a short outpatient program of IMT on inspiratory muscle performance, exercise capacity, perception of dyspnea, and the inspiratory fraction (IF. Thirty patients (24 males, 6 females with significant COPD (forced expiratory volume in one second [FEV1] = 46.21% ± 6.7% predicted, FEV1 = 33.6% ± 8.04% predicted were recruited for this study and had 3 months of IMT (30 minutes/day for 6 days/week in an outpatient clinic. Following IMT, there was a statistically significant increase in inspiratory muscle performance (an increase of the maximal inspiratory pressure from 59% ± 19.1% to 79% ± 21.85% predicted; p = 0.0342, a decrease in dyspnea (from 5.8 ± 0.78 to 1.9 ± 0.57; p = 0.0001, an increase in the distance walked during the 6 minute walk test, from 245 ± 52.37 m to 302 ± 41.30 m, and finally an increase in the IF (the new prognostic factor in COPD from 27.6 ± 9.7% to 31.4% ± 9.8%. The present study concludes that in patients with significant COPD, IMT results in improvement in performance, exercise capacity, sensation of dyspnea, and moreover an improvement in the IF prognostic factor. Keywords: inspiratory muscle training, dyspnea, inspiratory

In the framework of the African Monsoon Multidisciplinary Analyses (AMMA) programme, several tools have been developed in order to facilitate and speed up data and information exchange between researchers from different disciplines. The AMMA information system includes (i) a multidisciplinary user-friendly data management and dissemination system, (ii) report and chart archives associated with display websites and (iii) a scientific paper exchange system. The AMMA information system is enriched by several previous (IMPETUS...) and following projects (FENNEC, ESCAPE, QweCI, DACCIWA…) and is becoming a reference information system about West Africa monsoon. (i) The AMMA project includes airborne, ground-based and ocean measurements, satellite data use, modelling studies and value-added product development. Therefore, the AMMA database user interface enables to access a great amount and a large variety of data: - 250 local observation datasets, that cover many geophysical components (atmosphere, ocean, soil, vegetation) and human activities (agronomy, health). They have been collected by operational networks from 1850 to present, long term monitoring research networks (CATCH, IDAF, PIRATA...) or scientific campaigns; - 1350 outputs of a socio-economics questionnaire; - 60 operational satellite products and several research products; - 10 output sets of meteorological and ocean operational models and 15 of research simulations. All the data are documented in compliance with metadata international standards, and delivered into standard formats. The data request user interface takes full advantage of the data and metadata base relational structure and enables users to elaborate easily multicriteria data requests (period, area, property, property value…). The AMMA data portal counts around 800 registered users and process about 50 data requests every month. The AMMA databases and data portal have been developed and are operated jointly by SEDOO and ESPRI in France

Full Text Available Latife/letâif, meaning pleasant and nice statement humouring and cheering people, especially joking and humor, is conceptually an elusive and delicate meaning, a feeling coming to heart, or an amusing and weird statement and story. And letâif-nâme, derived from the word latife, can be composed of many different texts within itself (e.g. story, tale, joke and epigram, and can be written as a long-running story consisting of a separate text and a certain plot. The story Letâif-nâme, which is the subject of this paper and the author of which is not known, is an example of the latter kind of texts, and stands out as a work including eleven distinct stories / tales / jokes within a framework. This work was published in H.1268/ A.D.1851-52 in İstanbul. The work, comprising 153 pages, tells about the experiences of a hero called Yousuf Shah in the 17th century during the reign of Sultan Murad the fourth. This paper, focusing on the work Letâif-nâme, deals with the title, theme, summary, plot, characters, setting, time, narrator and the point of view of the work, and its linguistic and style characteristics within the framework of the story / novel analysis.
