Since its election, the Rudd Labor Government has created 10 new advisory bodies in the health portfolio, in addition to the 100 or more that were already established. An expansive and devolved advisory system could improve the health policy-making process, but only if it is integrated into the processes of government. We outline eight simple and practical measures that, if implemented, would make Australia's health advisory system more transparent and effective. Past experience shows that the most important factor governing the impact of health policy advisory bodies is political leadership.

Funding for radiation protection science and policy has been in decline for more than a decade. Agencies that set policies and standards for radiation protection depend on external expert groups for advice, and unless the funding situation is stabilized, the ability of these advisory organizations to provide timely advice will be compromised. This paper examines the history of radiation protection policy in the United States, reviews the funding patterns of international and national radiation protection advisory bodies, and suggests recommendations for assuring that radiological and radiation protection science remains an important part of the nation's public health policy agenda. Five major advisory organizations are the focus of this paper--ICRP, ICRU, NCRP, National Research Council BEIR Committees, and UNSCEAR. The recommendations developed in this paper address the following issues: (1) the need to coordinate activities among national and international advisory bodies in order to minimize overlap of work scope and ensure comprehensive coverage of major radiation protection issues; (2) the need to reevaluate activities and operations of advisory groups in the context of an ever-changing radiation protection landscape; and (3) the need to establish the NCRP as the major federal advisory organization for radiation protection in the United States and to stabilize funding through Congressional appropriations

This team includes one pediatrician, nurse, defectologist somatopaed and two psychologists.The work in the Advisory center is primarily preventive, and a part of it, curative. The preventive work includes medical examinations of children from 3 to 5 years of age. The work is organized by the psychologist who invites all the children from that age, born in a determined month on a level of Bitola. Beside the pediatrician survey also an psychological one is made.Gestalt therapy gives solutions, i. e. finished answers for solving the problem. The solution of the problem is gained by the patient himself during the therapy process, i. e. he becomes aware how to solve it.

Ellen V. Sigal, PhD, is Chairperson and Founder of Friends of Cancer Research (Friends), a think tank and advocacy organization based in Washington, DC. During the past 20 years, Friends has been instrumental in the creation and implementation of policies ensuring patients receive the best treatments in the fastest and safest way possible. Dr. Sigal is Chair of the inaugural board of directors of the Reagan-Udall Foundation, a partnership designed to modernize medical product development, accelerate innovation and enhance product safety in collaboration with the U.S. Food and Drug Administration. The vaccine landscape has changed considerably over the last decade with many new vaccines and technological developments, unprecedented progress in reaching out to children and the development of new financing mechanisms. At the same time, there are more demands and additional expectations of national policy makers, donors and other interested parties for increased protection through immunization. The Global Immunization Vision and Strategy (GIVS), which broadens the previous scope of immunization efforts, sets a number of goals to be met by countries. The WHO has recently reviewed and adjusted both its policy making structure and processes for vaccines and immunization to include an enlarged consultation process to generate evidence-based recommendations, thereby ensuring the transparency of the decision making process and improving communications. This article describes the process of development of immunization policy recommendations at the global level and some of their impacts. It focuses on the roles and modes of operating of the Strategic Advisory Group of Experts on immunization, which is the overarching advisory group involved with the issuance of policy recommendations, monitoring and facilitating the achievement of the GIVS goals. The article also describes the process leading to the publication of WHO vaccine position papers, which provide WHO recommendations on vaccine use. WHO vaccine-related recommendations have become a necessary step in the pathway to the introduction and use of vaccines, especially in developing countries and, consequently, have a clear and significant impact.

This document is the final report of the Fusion Policy Advisory Committee. The report conveys the Committee's views on the matters specified by the Secretary in his charge and subsequent letters to the Committee, and also satisfies the provisions of Section 7 of the Magnetic Fusion Energy Engineering Act of 1980, Public Law 96-386, which require a triennial review of the conduct of the national Magnetic Fusion Energy program. Three sub-Committee's were established to address the large number of topics associated with fusion research and development. One considered magnetic fusion energy, a second considered inertial fusion energy, and the third considered issues common to both. For many reasons, the promise of nuclear fusion as a safe, environmentally benign, and affordable source of energy is bright. At the present state of knowledge, however, it is uncertain that this promise will become reality. Only a vigorous, well planned and well executed program of research and development will yield the needed information. The Committee recommends that the US commit to a plan that will resolve this critically important issue. It also outlines the first steps in a development process that will lead to a fusion Demonstration Power Plant by 2025. The recommended program is aggressive, but we believe the goal is reasonable and attainable. International collaboration at a significant level is an important element in the plan

Volunteer citizen boards, such as Site Specific Advisory Boards, can be a very important key to success for the Department of Energy's (DOE's) Waste Management program. These boards can provide informed, independent recommendations reflecting the diversity of the community and its values. A successful volunteer process requires collaboration among regulators, DOE and other Boards; knowing how and when to interface with the broader public; understanding the diversity and representational issues of a citizens group; knowing the open-quotes ins and outsclose quotes of working with volunteers; education and training and most importantly, planning. Volunteers on a citizens board were created to tackle the big picture, policy decisions. The chair of the Rocky Flats Citizens Advisory Board will describe her Board's successes, including the challenges in reaching consensus agreements, as well as the need for integration with other boards and the sites' on-going public involvement programs to provide the input the department is seeking. This is not a systematic review, but aims to provide a representative cross-section of policy and advisory responses within set boundaries. There are a wide variety of approaches which I have categorized and tabulated ranging from ICNIRP/IEEE guidelines and "no extra precautions needed" to precautionary or scientific much lower maxima and extensive advice to minimize RF-EMF exposure, ban advertising/sale to children, and add exposure information to packaging. Precautionary standards use what I term an exclusion principle. The wide range of policy approaches can be confusing for parents/carers of children. Some consensus among advisory organizations would be helpful acknowledging that, despite extensive research, the highly complex nature of both RF-EMF and the human body, and frequent technological updates, means simple assurance of long-term safety cannot be guaranteed. Therefore, minimum exposure of children to RF-EMF is recommended. This does not indicate need for alarm, but mirrors routine health-and-safety precautions. Simple steps are suggested. ICNIRP guidelines need to urgently publish how the head, torso, and limbs' exposure limits were calculated and what safety margin was applied since this exposure, especially to the abdomen, is now dominant in many children.

Surgical site infections remain a common cause of morbidity, mortality, and increased length of stay and cost amongst hospitalized patients in the United States. This article summarizes the evidence used to inform the Centers for Disease Control and Prevention and the Healthcare Infection Control Practices Advisory Committee Guideline for the Prevention of Surgical Site Infection (2017), and highlights key updates and new recommendations. We also present specific suggestions for how infection preventionists can play a central role in guideline implementation by translating these recommendations into evidence-based policies and practices in their facility. Full Text Available Lately, Internet constitutes a major tool for transactions in every aspect and supports innovative marketing policies. Broadband Internet has become “the key to success” for businesses, as it offers various advantages and benefits through Internet marketing (e-marketing policies. In Greece, mountainous areas are usually covered with snow during winter months; so, skiing centers have become an important asset for winter tourism. The Internet evolution and the development of network infrastructure enhance marketing policies for winter tourism activities. This paper studies the use of marketing policies in Greek skiing centers through the Internet, such as promotional activities, website interactivity, accommodation & entertainment information, online weather forecast, guest book, etc Therefore, the paper aims to optimize and evaluate skiing centers in Greece, qualitatively and quantitatively according to e-marketing policies used as criteria, based on the multicriteria method of PROMETHEE II and further to classify them in groups. Finally we identify and describe the optimum group of skiing centers to be used as a model with enhanced customer communication services.

Developing Child-Centered Social Policies: When Professionalism Takes Over

Full Text Available No nation today can be understood as being fully child-centered, but many are pursuing social policies heavily favoring children. The emphasis on individual rights and the growth of scientific knowledge underpinning many of these policies have led to the improvement of the lives of a great many children. Paradoxically, these same knowledge bases informing social policies often produce representations and images of children and their parents that are detrimental for both of these groups. Using Norwegian child welfare policies and practices as examples, I will examine some of the possible pitfalls of child-centered praxis. The key question here is one asking whether the scientific frame central to child welfare professionalism has positioned children and parents as objects rather than subjects in their own lives and, in so doing, required them to live up to standards of life defined for them by experts. A central question will involve exploring the extent to which scientific knowledge has erased political and ethical considerations from the field when assessing social problems.

Conflicts of interest (COI) in research are an important emerging topic of investigation and are frequently cited as a serious threat to the integrity of human participant research. To study financial conflicts of interest (FCOI) policies for individual investigators working in Canadian academic health centers. Survey instrument containing 61 items related to FCOI. All Canadian academic health science centers (universities with faculties of medicine, faculties of medicine and teaching hospitals) were requested to provide their three primary FCOI policies. Number of all centers and teaching hospitals with policies addressing each of the 61 items related to FCOI. Only one item was addressed by all 74 centers. Thirteen items were present in fewer than 25% of centers. Fewer than one-quarter of hospitals required researchers to disclose FCOI to research participants. The role of research ethics boards (REBs) in hospitals was marginal. Asking centers to identify only three policies may not have inclusively identified all FCOI policies in use. Additionally, policies at other levels might apply. For instance, all institutions receiving federal grant money must comply with the Tri-Council Policy Statement: Ethical Conduct for Research Involving Humans. Canadian centers within the same level (for instance, teaching hospitals) differ significantly in the areas that their policies address and these policies differ widely in their coverage. Presently, no single policy in any Canadian center informs researchers about the broad range of individual FCOI issues. Canadian investigators need to understand the environment surrounding FCOI, be able to access and follow the relevant policies and be confident that they can avoid entering into a FCOI.

Background: In recent times, there has been a growing demand internationally for health policies to be based on reliable research evidence. Consequently, there is a need to strengthen institutions and mechanisms that can promote interactions among researchers, policy-makers and other stakeholders who can influence the uptake of research findings. The Health Policy Advisory Committee (HPAC) is one of such mechanisms that can serve as an excellent forum for the interaction of policy-makers and researchers. Therefore, the need to have a long term mechanism that allows for periodic interactions between researchers and policy-makers within the existing government system necessitated our implementation of a newly established HPAC in Ebonyi State Nigeria, as a Knowledge Translation (KT) platform. The key study objective was to enhance the capacity of the HPAC and equip its members with the skills/competence required for the committee to effectively promote evidence informed policy-making and function as a KT platform. Methods: A series of capacity building programmes and KT activities were undertaken including: i) Capacity building of the HPAC using Evidence-to-Policy Network (EVIPNet) SUPPORT tools; ii) Capacity enhancement mentorship programme of the HPAC through a three-month executive training programme on health policy/health systems and KT in Ebonyi State University Abakaliki; iii) Production of a policy brief on strategies to improve the performance of the Government’s Free Maternal and Child Health Care Programme in Ebonyi State Nigeria; and iv) Hosting of a multi-stakeholders policy dialogue based on the produced policy brief on the Government’s Free Maternal and Child Health Care Programme. Results: The study findings indicated a noteworthy improvement in knowledge of evidence-to-policy link among the HPAC members; the elimination of mutual mistrust between policy-makers and researchers; and an increase in the awareness of importance of HPAC in the Ministry

Korean experts are currently participating in most of these advisory groups, but there is something yet to improve especially in faithful reflection of Korean nuclear policy as well as a systematic and effective utilization technical information received in the committee meetings. Therefore, it is necessary firstly to analyze results and systematic follow-up of technology developments for each IAEA committee group to solidate activities of Korean members. Based on these analyses, we can hence support administratively Korean members to do faithful reflection of Korean nuclear policy. In addition, a scheme for systematic management of the information should be drawn up for use of these information effectively for nuclear power projects, R and D, safety regulation, and establishment of nuclear policy in Korea

Full Text Available Cloud computing depends on sharing distributed computing resources to handle different services such as servers, storage and applications. The applications and infrastructures are provided as pay per use services through data center to the end user. The data centers are located at different geographic locations. However, these data centers can get overloaded with the increase number of client applications being serviced at the same time and location; this will degrade the overall QoS of the distributed services. Since different user applications may require different configuration and requirements, measuring the user applications performance of various resources is challenging. The service provider cannot make decisions for the right level of resources. Therefore, we propose a Variable Service Broker Routing Policy – VSBRP, which is a heuristic-based technique that aims to achieve minimum response time through considering the communication channel bandwidth, latency and the size of the job. The proposed service broker policy will also reduce the overloading of the data centers by redirecting the user requests to the next data center that yields better response and processing time. The simulation shows promising results in terms of response and processing time compared to other known broker policies from the literature.

Although some researchers have examined nutrition and physical activity policies within urban child care centers, little is known about the potentially unique needs of rural communities. Child care centers serving preschool children located within low-income rural communities (n = 29) from seven states (Indiana, Kansas, Michigan, North Dakota, Ohio, South Dakota, and Wisconsin) were assessed to determine current nutrition and physical activity (PA) practices and policies. As part of a large-scale childhood obesity prevention project, the Community Healthy Living Index's previously validated Early Childhood Program Assessment Tool was used to collect data. Descriptive statistical analysis was conducted to identify high-priority areas. Healthy People 2020 and the Academy of Nutrition and Dietetics' recommendations for nutrition and PA policies in child care centers were used as benchmarks. Reports of not fully implementing (nutrition-related policies or practices within rural early child care centers were identified. Centers not consistently serving a variety of fruits (48%), vegetables (45%), whole grains (41%), limiting saturated fat intake (31%), implementing healthy celebration guidelines (41%), involving children in mealtime (62%), and referring families to nutrition assistance programs (24%) were identified. More than one third of centers also had limited structured PA opportunities. Although eligible, only 48% of the centers participated in the Child and Adult Care Food Program. Overall, centers lacked parental outreach, staff training, and funding/resources to support nutrition and PA. These results provide insight into where child care centers within low-income, rural communities may need assistance to help prevent childhood obesity.

Full Text Available Abstract Background Person-centered care is a critical component of quality care, essential to enable treatment adherence, and maximize health outcomes. Improving the quality of health services is a key strategy to achieve the new global target of zero preventable maternal deaths by 2030. Recognizing this, the Government of India has in the last decade initiated a number of strategies to address quality of care in health and family welfare services. Methods We conducted a policy review of quality improvement strategies in India from 2005 to 15, covering three critical areas– maternal and newborn health, family planning, and abortion (MNHFP + A. Based on Walt and Gilson’s policy triangle framework, we analyzed the extent to which policies incorporated person-centered care, while identifying unaddressed issues. Data was sourced from Government of India websites, scientific and grey literature databases. Results Twenty-two national policy documents, comprising two policy statements and 20 implementation guidelines of specific schemes were included in the review. Quality improvement strategies span infrastructure, commodities, human resources, competencies, and accountability that are driving quality assurance in MNHFP + A services. However, several implementation challenges have affected compliance with person-centered care, thereby affecting utilization and outcomes. Conclusion Focus on person-centered care in Indian MNHFP + A policy has increased in recent years. Nevertheless, some aspects must still be strengthened, such as positive interpersonal behavior, information sharing and promptness of care. Implementation can be improved through better provider training, patient feedback and monitoring mechanisms. Moreover, unless persisting structural challenges are addressed implementation of person-centered care in facilities will not be effective.

An international nanoscience advisory board to improve and harmonize nanotechnology oversight

As governments around the world begin to implement regulations aimed at controlling nanotechnology, those regulations should be based upon the best available science, applied as consistently as possible within jurisdictions and, to the extent feasible, across jurisdictions. These goals would be easier to achieve with the creation of an international nanoscience advisory board. Such a body could be modeled on similar international scientific advisory bodies for other issues, such as the Intergovernmental Panel on Climate Change (IPCC) and the Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services. Such a body should also take into account lessons learned from these similar organizations. An international nanoscience advisory board could assist regulatory bodies by providing a central source of accurate scientific information about the risks and benefits of nanotechnology, including relevant uncertainties, rather than having each regulatory body make these determinations independently. An international nanoscience advisory board could facilitate harmonization within and between jurisdictions by involving the top experts in the field to produce a centralized knowledge base for regulatory decisions. While an international nanoscience advisory board presents many potential benefits, it also faces significant difficulties, which are best illustrated by examining the history and challenges of existing international science advisory bodies.

In an effort to regulate physician conflicts of interest, some US academic medical centers (AMCs) enacted policies restricting pharmaceutical representative sales visits to physicians (known as detailing) between 2006 and 2012. Little is known about the effect of these policies on physician prescribing. To analyze the association between detailing policies enacted at AMCs and physician prescribing of actively detailed and not detailed drugs. The study used a difference-in-differences multivariable regression analysis to compare changes in prescribing by physicians before and after implementation of detailing policies at AMCs in 5 states (California, Illinois, Massachusetts, Pennsylvania, and New York) that made up the intervention group with changes in prescribing by a matched control group of similar physicians not subject to a detailing policy. The monthly within-drug class market share of prescriptions written by an individual physician for detailed and nondetailed drugs in 8 drug classes (lipid-lowering drugs, gastroesophageal reflux disease drugs, diabetes drugs, antihypertensive drugs, hypnotic drugs approved for the treatment of insomnia [sleep aids], attention-deficit/hyperactivity disorder drugs, antidepressant drugs, and antipsychotic drugs) comparing the 10- to 36-month period before implementation of the detailing policies with the 12- to 36-month period after implementation, depending on data availability. The analysis included 16 121 483 prescriptions written between January 2006 and June 2012 by 2126 attending physicians at the 19 intervention group AMCs and by 24 593 matched control group physicians. The sample mean market share at the physician-drug-month level for detailed and nondetailed drugs prior to enactment of policies was 19.3% and 14.2%, respectively. The antibiotic policy is the set of strategies and activities undertaken to organize the antimicrobial treatment in the hospital, and achieve health outcomes for patients. The basic principles are to be direct evidence-based medicine, local epidemiology and freedom for prescribing physicians. An antibiotic policy is now more necessary than ever for clinical, epidemiological and economic reasons. The Infection Committee is responsible for the antibiotics policy in hospitals. Its functions as an advisory body to the medical directorate are the analysis of the epidemiology of the infections in the center, measures for its prevention and control, improving the appropriate use of antimicrobials, training, and knowledge production. To achieve clinical, environmental and economic policy objectives of antibiotics is not easy. The agreement of hundreds of professionals for recommendations on indications, dosage and duration of antibiotic treatment, based on the best scientific evidence and local guides is complex, but it can be done. The key to this is that the Infection Committee develops antimicrobial stewardship through a multidisciplinary team and professional leadership, and has the institutional support to ensure that the proper use of antimicrobials is a priority for the center, and therefore of each of the services involved, and that the team has the resources for antimicrobial stewardship. Full Text Available The nature of policy advisory systems and the capacity and influence of individual system actors has been a subject of much interest in recent years, especially vis-à-vis observed trends towards the twin themes of politicization and externalization of policy advice. Studies to date for the most part have focused only on the capacity of highly visible advisory system actors such as professional policy analysts in government or those in the NGO and business sectors. This study examines the role of the ‘shadow’ or ‘invisible’ actors employed by governments on temporary contracts as managerial or other kinds of policy consultants to undertake activities related to policy development and evaluation processes. The study reports on the findings of a 2012-2013 survey of such consultants in Canada and presents data on relevant aspects of their background, training, perceptions and capabilities compared to permanent policy analysts employed fulltime by governments. It finds most consultants to be better qualified than their permanent counterparts and to primarily engage, like the latter, in process-related policy work. This answers some questions about the roles and relationships of these members of the advisory system but raises other questions about where the ‘substance’ of policies originates.

National Immunization Technical Advisory Groups (NITAGs) provide independent, evidence-informed advice to assist their governments in immunization policy formation. However, many NITAGs face challenges in fulfilling their roles. Hence the many requests for formation of a network linking NITAGs together so they can learn from each other. To address this request, the Health Policy and Institutional Development (HPID) Center (a WHO Collaborating Center at the Agence de Médecine Préventive - AMP), in collaboration with WHO, organized a meeting in Veyrier-du-Lac, France, on 11 and 12 May 2016, to establish a Global NITAG Network (GNN). The meeting focused on two areas: the requirements for (a) the establishment of a global NITAG collaborative network; and (b) the global assessment/evaluation of the performance of NITAGs. 35 participants from 26 countries reviewed the proposed GNN framework documents and NITAG performance evaluation. Participants recommended that a GNN should be established, agreed on its governance, function, scope and a proposed work plan as well as setting a framework for NITAG evaluation. Copyright © 2017.

The Center for Energy Policy and Research of the New York Institute of Technology has organized an ENERGY ADVISORY SERVICE for information dissemination on energy conservation techniques to various target audiences in the New York, New Jersey, and Connecticut area. The Service, in operation since June 1, 1976 established four channels of communication to disseminate energy conservation information and technical assistance: The NYIT Energy Hot Line, the NYIT/ERDA (MRC-TV) Energy Management Seminar program, the Energy Information Center, and the Referral Service. The purposes of this report are two-fold: (1) to provide expanded and up-dated information on operating experiences associated with the NYIT Energy Hot Line and the NYIT/ERDA (MRC-TV) Energy Management Seminar Program, and (2) to provide recently analyzed data emerging from a study designed to investigate the two channels for their absolute and comparative effectiveness in energy technology transfer. Specifically research was designed to cast light on the following questions: (a) in terms of demographics and prior energy attitudes, what kinds of people tend to use the hot line or be drawn to the television seminar; (b) how did channel users feel about the channel after experiencing it; (c) how did channel users perform on energy-transfer effectiveness measures of knowledge and intention to act; and (d) was there any relationship between performance or effectiveness measures and selected demographic and attitudinal variables associated with channel users.

Irrigation agriculture is one the biggest consumer of water in Europe, especially in southern regions, where it accounts for up to 70% of the total water consumption. The EU Common Agricultural Policy, combined with the Water Framework Directive, imposes to farmers and irrigation managers a substantial increase of the efficiency in the use of water in agriculture for the next decade. Irrigating according to reliable crop water requirement estimates is one of the most convincing solution to decrease agricultural water use. Here we present an innovative irrigation advisory service, applied in Campania region (Southern Italy), where a satellite assisted irrigation advisory service has been operating since 2006. The advisory service is based on the optimal combination of VIS-NIR high resolution satellite images (Landsat, Deimos, Rapideye) to map crop vigour, and high resolution numerical weather prediction for assessing the meteorological variables driving the crop water needs in the short-medium range. The advisory service is broadcasted with a simple and intuitive web app interface which makes daily real time irrigation and evapotranspiration maps and customized weather forecasts (based on Cosmo Leps model) accessible from desktop computers, tablets and smartphones.

Substantial controversy is a consistent feature of UK animal health and welfare policy. BSE, foot and mouth disease, bovine TB and badger culling, large indoor dairies, and wild animals in circuses are examples. Such policy issues are inherently normative; they include a substantial moral dimension. This paper reviews UK animal welfare advisory bodies such as the Animal Health and Welfare Board of England, the Farm Animal Welfare Council and the Animals in Science Committee. These bodies play a key advisory role, but do not have adequate expertise in ethics to inform the moral dimension of policy. We propose an "Ethics Council for Animal Policy" to inform the UK government on policy that significantly impacts sentient species. We review existing Councils (e.g., the Nuffield Council on Bioethics and The Netherlands Council on Animal Affairs) and examine some widely used ethical frameworks (e.g., Banner's principles and the ethical matrix). The Ethics Council for Animal Policy should be independent from government and members should have substantial expertise in ethics and related disciplines. A pluralistic six-stage ethical framework is proposed: (i) Problematisation of the policy issue, (ii) utilitarian analysis, (iii) animal rights analysis, (iv) virtue-based analysis, (v) animal welfare ethic analysis, and (vi) integrated ethical analysis. The paper concludes that an Ethics Council for Animal Policy is necessary for just and democratic policy making in all societies that use sentient nonhuman species.

This study examined the roles academic researchers can play to inform policy and environmental strategies that promote health and prevent disease. Prevention Research Centers (PRCs) engage in academic-community partnerships to conduct applied public health research. Interviews were used to collect data on the roles played by 32 PRCs to inform policy and environmental strategies that were implemented between September 2009 and September 2010. Descriptive statistics were calculated in SAS 9.2. A difference in roles played was observed depending on whether strategies were policy or environmental. Of the policy initiatives, the most common roles were education, research, and partnership. In contrast, the most prevalent roles the PRCs played in environmental approaches were research and providing health promotion resources. Academic research centers play various roles to help inform policy and environmental strategies. © 2014 Society for Public Health Education.

The Independent Payment Advisory Board (IPAB) is a vastly powerful component of the president's health care reform law, with authority to issue recommendations to reduce the growth in Medicare spending, providing recommendations to be considered by Congress and implemented by the administration on a fast track basis. Ever since its inception, IPAB has been one of the most controversial issues of the Patient Protection and Affordable Care Act (ACA), even though the powers of IPAB are restricted and multiple sectors of health care have been protected in the law. IPAB works by recommending policies to Congress to help Medicare provide better care at a lower cost, which would include ideas on coordinating care, getting rid of waste in the system, providing incentives for best practices, and prioritizing primary care. Congress then has the power to accept or reject these recommendations. However, Congress faces extreme limitations, either to enact policies that achieve equivalent savings, or let the Secretary of Health and Human Services (HHS) follow IPAB's recommendations. IPAB has strong supporters and opponents, leading to arguments in favor of or against to the extreme of introducing legislation to repeal IPAB. The origins of IPAB are found in the ideology of the National Institute for Health and Clinical Excellence (NICE) and the impetus of exploring health care costs, even though IPAB's authority seems to be limited to Medicare only. The structure and operation of IPAB differs from Medicare and has been called the Medicare Payment Advisory Commission (MedPAC) on steroids. The board membership consists of 15 full-time members appointed by the president and confirmed by the Senate with options for recess appointments. The IPAB statute sets target growth rates for Medicare spending. The applicable percent for maximum savings appears to be 0.5% for year 2015, 1% for 2016, 1.25% for 2017, and 1.5% for 2018 and later. Managed care and governmental policies have restructured hospital reimbursement. We examined reimbursement trends in trauma care to assess the impact of this market driven change on an urban academic health center. Patients injured between January 1997 and December 1999 were analyzed for Injury Severity Score (ISS), length of hospital stay, hospital cost, payer, and reimbursement. Between 1997 and 1999, the volume of patients with an ISS less than 9 increased and length of stay decreased. In addition, overall cost, payment, and profit margin increased. Commercially insured patients accounted for this margin increase, because the margins of managed care and government insured patients experienced double-digit decreases. Patients with ISS of 9 or greater also experienced a volume increase and a reduction in length of stay; however, costs within this group increased greater than payments, thereby reducing profit margin. Whereas commercially insured patients maintained their margin, managed care and government insured patients did not (double- and triple-digit decreases). Managed care and current governmental policies have a negative impact on urban academic health center reimbursement. Commercial insurers subsidize not only the uninsured but also the government insured and managed care patients as well. National awareness of this issue and policy action are paramount to urban academic health centers and may also benefit commercial insurers.

The Environmental Management Site Specific Advisory Board (EM SSAB) is comprised of eight Citizens' Advisory Boards, chartered under the Federal Advisory Committee Act (FACA) by the Assistant Secretary for Environmental Management for the U. S. Department of Energy (DOE). The purpose of the EM SSAB is to provide the DOE with recommendations regarding Environmental Management issues from legacy waste produced at major sites across the DOE Complex. The Northern New Mexico Citizens' Advisory Board (NNMCAB) is the site specific advisory board to the DOE on issues of environmental monitoring, remediation, waste management and long-term stewardship at Los Alamos National Laboratory (LANL). The other boards are located at: Hanford, Idaho, Nevada, Paducah, Portsmouth, Oak Ridge and Savannah River. Using broad citizen outreach and input, the SSAB provides an official mechanism for the citizenry at each of these sites to monitor current activities affecting the region and also to have input into the prioritization of future activities. 'The mission of the EM SSAB is to more directly involve stakeholders in EM Planning and decision-making processes for the nuclear weapons complex cleanup. DOE has various means of involving the public in its planning and decision-making processes; the EM SSAB is only one component of EM's public participation program, and is not intended to be an exclusive means of public participation. It is the policy of DOE and EM to conduct it programs in an open and responsive manner, thereby encouraging and providing the opportunity for public participation in its planning and decision-making processes. This presentation describes the tools and services provided by the National Atmospheric Release Advisory Center (NARAC) at Lawrence Livermore National Laboratory (LLNL) for modeling the impacts of airborne hazardous materials. NARAC provides atmospheric plume modeling tools and services for chemical, biological, radiological, and nuclear airborne hazards. NARAC can simulate downwind effects from a variety of scenarios, including fires, industrial and transportation accidents, radiation dispersal device explosions, hazardous material spills, sprayers, nuclear power plant accidents, and nuclear detonations. NARAC collaborates on radiological dispersion source terms and effects models with Sandia National Laboratories and the U.S. Nuclear Regulatory Commission. NARAC was designated the interim provider of capabilities for the Department of Homeland Security's Interagency Modeling and Atmospheric Assessment Center by the Homeland Security Council in April 2004. The NARAC suite of software tools include simple stand-alone, local-scale plume modeling tools for end-user's computers, and Web- and Internet-based software to access advanced modeling tools and expert analyses from the national center at LLNL. Initial automated, 3-D predictions of plume exposure limits and protective action guidelines for emergency responders and managers are available from the center in 5-10 minutes. These can be followed immediately by quality-assured, refined analyses by 24 x 7 on-duty or on-call NARAC staff. NARAC continues to refine calculations using updated on-scene information, including measurements, until all airborne releases have stopped and the hazardous threats are mapped and impacts assessed. Model predictions include the 3-D spatial and time-varying effects of weather, land use, and terrain, on scales from the local to regional to global. Initial warning center decisions are based on an earthquake's location, magnitude, depth, distance from coastal locations, and precomputed threat estimates based on tsunami models computed from similar events. The new criteria will help limit the geographical extent of warnings and advisories to threatened regions, and complement the new operational tsunami product suite. Criteria are set for tsunamis generated by earthquakes, which are by far the main cause of tsunami generation (either directly through sea floor displacement or indirectly by triggering of sub-sea landslides).The new criteria require development of a threat data base which sets warning or advisory zones based on location, magnitude, and pre-computed tsunami models. The models determine coastal tsunami amplitudes based on likely tsunami source parameters for a given event. Based on the computed amplitude, warning and advisory zones are pre-set.

Putting science at the heart of European policy

One year ago, the incoming European Commission President Jean-Claude Juncker shocked the scientific world by scrapping the post of Chief Scientific Advisor. This week, the Commission made amends by launching a well-considered Scientific Advisory Mechanism (SAM) that not only puts science back at the heart of policy, but does so in a much more structured and robust way than conferring such responsibility on a single individual. The SAM has two independent strands: an advisory group of seven scientists, and funding through the Horizon 2020 programme for national academies and learned societies to network and collaborate on policy issues. Both are backed up by a secretariat at Commission headquarters in Brussels. Surgical site infection (SSI) is a common type of health-care-associated infection (HAI) and adds considerably to the individual, social, and economic costs of surgical treatment. This document serves to introduce the updated Guideline for the Prevention of SSI from the Centers for Disease Control and Prevention (CDC) and the Healthcare Infection Control Practices Advisory Committee (HICPAC). The Core section of the guideline addresses issues relevant to multiple surgical specialties and procedures. The second procedure-specific section focuses on a high-volume, high-burden procedure: Prosthetic joint arthroplasty. While many elements of the 1999 guideline remain current, others warrant updating to incorporate new knowledge and changes in the patient population, operative techniques, emerging pathogens, and guideline development methodology.

Full Text Available In this paper the catalyzing influence of Bill C-61—a proposed amendment to the Canadian Copyright Act—and other recent ICT-related policy developments on activism in Canada is examined. The discussion expounds upon the role of academics and activists in fostering a broader public discourse about ICT policy, with attention being given to three key moments in Canadian communication policy: the development of the “information highway” in the mid-1990s and, in particular, the activities of the Information Highway Advisory Council (IHAC; the creation of the Telecommunications Policy Review Panel (TPRP in 2005; and the current debates regarding the issue of net neutrality. The analysis demonstrates how “esoteric” digital policy issues are now seen by many Canadians as worthy of their energies. This suggests that politicians cannot afford to ignore their constituents’ concerns about such policy issues as traffic shaping, throttling, fair dealings, and anti-circumvention measures. And, likewise, that academics working in the realm of communication policy domain would do well not to overlook the role of citizens, grassroots groups and non-profit organizations in actively seeking a voice in the various structures of policymaking.

Full Text Available The purpose of this study was to determine the characteristics and motivational factors of effective Extension advisory leaders. This Delphi study was conducted with a selected group of County Extension Directors and a group of Extension State Advisory Leaders. The study identified 10 characteristics that distinguish an effective Extension advisory leader. Some of these characteristics are explicit and easy to observe, while others are implicit and difficult to directly observe. Therefore, it is practical to use directly observable characteristics of effective advisory leaders when selecting volunteers. Once potential volunteers are spotted in the community, implicit characteristics of effective advisory leaders should be used to further screen them before they are selected. The study also identified the eight most important factors motivating individuals to volunteer as effective advisory leaders. Understanding these motivational factors is helpful for creating an environment for attracting and retaining effective volunteers. Understanding their motivation for volunteer work and creating an environment for them to meet the motivating factors for volunteering will lead to volunteer satisfaction and retention. The findings of this study can be used to build strong Extension advisory councils.

Full Text Available The article is focused on the analysis of organizational and legal conditions for the functioning of the highest advisory bodies in the fields of research and higher education, created at the state level at the central governing institutions in Ukraine and selected European countries. The article is aimed at the research of the model of formation and functioning of the national advisory bodies on research and higher education in Poland, Germany, and Czech Republic in the system of science management, based on the national legislation. The following national bodies are studied in the paper: National Council of Science and Higher Education of Poland, the Committee of Science Policy of Poland, the Council for Research, Development and Innovation of Czech Republic, National Council for Research, Innovation and Science Policy of Hungary, the German Research Council and others. It is proved that advisory bodies are an important element of the system of public governance, as well as actors in shaping of the national science and higher education policy. They are formed in a democratic way due to the principle of shared governance and mostly of institutional representation. The article considered selected aspects of the establishment of the National R&D Council of Ukraine, as stated in the Law of Ukraine «On Scientific and R&D Activity» comparing with the relevant European institutions. The analysis of the concept for the National Council for Science and Technologies (R&D of Ukraine along with the administrative and management risks could be considered while implementation of the Law of Ukraine «On Scientific and R&D Activity».

This presentation describes the tools and services provided by the National Atmospheric Release Advisory Center (NARAC) at Lawrence Livermore National Laboratory (LLNL) for modeling the impacts of airborne hazardous materials. NARAC provides atmospheric plume modeling tools and services for chemical, biological, radiological, and nuclear airborne hazards. NARAC can simulate downwind effects from a variety of scenarios, including fires, industrial and transportation accidents, radiation dispersal device explosions, hazardous material spills, sprayers, nuclear power plant accidents, and nuclear detonations. NARAC collaborates on radiological dispersion source terms and effects models with Sandia National Laboratories and the U.S. Nuclear Regulatory Commission. NARAC was designated the interim provider of capabilities for the Department of Homeland Security's Interagency Modeling and Atmospheric Assessment Center by the Homeland Security Council in April 2004. The NARAC suite of software tools include simple stand-alone, local-scale plume modeling tools for end-user's computers, and Web- and Internet-based software to access advanced modeling tools and expert analyses from the national center at LLNL. Initial automated, 3-D predictions of plume exposure limits and protective action guidelines for emergency responders and managers are available from the center in 5-10 minutes. These can be followed immediately by quality-assured, refined analyses by 24 x 7 on-duty or on-call NARAC staff. NARAC continues to refine calculations using updated on-scene information, including measurements, until all airborne releases have stopped and the hazardous threats are mapped and impacts assessed. Model predictions include the 3-D spatial and time-varying effects of weather, land use, and terrain, on scales from the local to regional to global. This article examines the process of rebuilding institutional trust in the Canadian blood system in the aftermath of the tainted blood scandal. Our focus is the policy of lifetime deferral from donating blood for men who have sex with men. Drawing on findings from interviews with representatives of Health Canada's Expert Advisory Committee on Blood Regulation, the National Liaison Committee, Canadian Blood Services, and blood consumer groups, we demonstrate how claims making about rights, discrimination, science, and risk contribute to policy continuity. We also examine the link between policy continuity and the management of reputational risk.

Full Text Available Financial conflicts of interest exist between industry and physicians, and these relationships have the power to influence physicians’ medical practice. Transparency about conflicts matters for ensuring adequate informed consent, controlling healthcare expenditure, and encouraging physicians’ reflection on professionalism. The US Centers for Medicare & Medicaid Services (CMS launched the Open Payments Program (OPP to publicly disclose and bring transparency to the relationships between industry and physicians in the United States. We set out to explore user awareness of the database and the ease of accessibility to disclosed information, however, as we show, both awareness and actual use are very low. Two practical policies can greatly enhance its intended function and help alleviate ethical tension. The first is to provide data for individual physicians not merely in absolute terms, but in meaningful context, that is, in relation to the zip code, city, and state averages. The second increases access to the OPP dataset by adding hyperlinks from physicians’ professional websites directly to their Open Payments disclosure pages. These changes considerably improve transparency and the utility of available data, and can furthermore enhance professionalism and accountability by encouraging physicians to reflect more actively on their own practices.

Health at the center of health systems reform: how philosophy can inform policy.

Contemporary views hold that health and disease can be defined as objective states and thus should determine the design and delivery of health services. Yet health concepts are elusive and contestable. Health is neither an individual construction, a reflection of societal expectations, nor only the absence of pathologies. Based on philosophical and sociological theory, empirical evidence, and clinical experience, we argue that health has simultaneously objective and subjective features that converge into a dynamic complex-adaptive health model. Health (or its dysfunction, illness) is a dynamic state representing complex patterns of adaptation to body, mind, social, and environmental challenges, resulting in bodily homeostasis and personal internal coherence. The "balance of health" model-emergent, self-organizing, dynamic, and adaptive-underpins the very essence of medicine. This model should be the foundation for health systems design and also should inform therapeutic approaches, policy decision-making, and the development of emerging health service models. A complex adaptive health system focused on achieving the best possible "personal" health outcomes must provide the broad policy frameworks and resources required to implement people-centered health care. People-centered health systems are emergent in nature, resulting in locally different but mutually compatible solutions across the whole health system.

Automated procedure tracking systems have been of interest in the nuclear power industry due to the huge numbers of documented procedures required for the operation and maintenance of commercial power reactors. Tools are under development at Ames Research center that form the basis of an intelligent advisory system for process control applications. Two tools, one for documentation maintenance and management and the other for automated fault diagnosis, are being integrated to build an advisory system appropriate for use in facilities that require emergency readiness and safety-critical operations. The problem of automation of documents management and maintenance has been analyzed from artificial intelligence and from human factors viewpoints. A software tool, called computer-integrated documentation (CID), has been designed and is under further development to address these issues. A fault diagnosis algorithm has been developed and implemented in a software tool called the fault-tree diagnosis system (FTDS). The algorithm uses a knowledge base that is easily changed and updated to reflect current system status. The 2013 Institute of Medicine (IOM) report "Sodium Intake in Populations: Assessment of Evidence" did not support the current recommendations of the IOM and the American Heart Association (AHA) to reduce daily dietary sodium intake to below 2,300 mg. The report concluded that the population-based health outcome evidence was not sufficient to define a safe upper intake level for sodium. Recent studies have extended this conclusion to show that a sodium intake below 2,300 mg/day is associated with increased mortality. In spite of this increasing body of evidence, the AHA, Centers for Disease Control (CDC), other public health advisory bodies, and major medical journals have continued to support the current policy of reducing dietary sodium.

Data from a utility-company-sponsored telephone survey of households on Long Island, New York, where the Shoreham Nuclear Power Station is located, are used in this paper to test this proposition. Analyses of responses given to three sets of increasingly serious reactor accident scenarios, with and without information instructing people what protective actions to take, raise serious questions about the ability of emergency notification messages to affect human behavior in a radiological emergency. The results suggest firstly, that even if people are specifically advised not to evacuate, most would be inclined to do so; and secondly, that attempts to stifle the propensity to evacuate among those who are not at risk are likely to increase the propensity to stay behind among those who are at risk and should evacuate

This report sets out the Radioactive Waste Management Advisory Committee's (RWMAC's) view of the way in which policy for the long-term management of the United Kingdom's (UK's) solid radioactive waste should be developed. It does not cover subsequent policy implementation stages, which will need to be the subject of separate consideration. It is RWMAC's view that 'do nothing' or 'decide-announce-defend' approaches to policy formulation are inappropriate, and what is needed is a consensus-building approach involving full and open discussion of the issues. Such an approach now offers the best chance of identifying a policy that can ultimately be delivered. This process must be founded on a set of clear guiding principles. These should be: provision of adequate time for exploration and resolution of complex issues; early involvement of the public and other stakeholders; openness and transparency; a deliberative and accessible process in decision-making; and commitment to appropriate peer review of scientific and other expert input. The issue of equity, that is the extent to which both the process for deciding policy and the policy itself are generally perceived and accepted to be fair, will also need to be explored. The process should provide for all the practicable options for the long-term management of the UK's radioactive waste to be assessed against a set of common evaluation criteria. Both the list of practicable options and the criteria against which they are evaluated should be developed through a process of open discussion.

Advancing team-based primary health care: a comparative analysis of policies in western Canada.

We analyzed and compared primary health care (PHC) policies in British Columbia, Alberta and Saskatchewan to understand how they inform the design and implementation of team-based primary health care service delivery. The goal was to develop policy imperatives that can advance team-based PHC in Canada. We conducted comparative case studies (n = 3). The policy analysis included: Context review: We reviewed relevant information (2007 to 2014) from databases and websites. Policy review and comparative analysis: We compared and contrasted publically available PHC policies. Key informant interviews: Key informants (n = 30) validated narratives prepared from the comparative analysis by offering contextual information on potential policy imperatives. Advisory group and roundtable: An expert advisory group guided this work and a key stakeholder roundtable event guided prioritization of policy imperatives. The concept of team-based PHC varies widely across and within the three provinces. We noted policy gaps related to team configuration, leadership, scope of practice, role clarity and financing of team-based care; few policies speak explicitly to monitoring and evaluation of team-based PHC. We prioritized four policy imperatives: (1) alignment of goals and policies at different system levels; (2) investment of resources for system change; (3) compensation models for all members of the team; and (4) accountability through collaborative practice metrics. Policies supporting team-based PHC have been slow to emerge, lacking a systematic and coordinated approach. Greater alignment with specific consideration of financing, reimbursement, implementation mechanisms and performance monitoring could accelerate systemic transformation by removing some well-known barriers to team-based care.

Tobacco Pricing in Military Stores: Views of Military Policy Leaders.

Higher tobacco taxes reduce tobacco use. On military installations, cigarettes and other tobacco products are sold tax-free, keeping prices artificially low. Pricing regulations in the military specify that tobacco should be within 5% of the local most competitive price, but prices still average almost 13% lower than those at local Walmarts. To gain insight into policy leaders' ideas and positions on military tobacco pricing, we interviewed members of the Department of Defense (DoD) Addictive Substances Misuse Advisory Committee and the Advisory Committee on Tobacco about tobacco pricing policies (n = 12). Participants frequently lacked specific knowledge of details of military pricing policy, and the impact higher prices might have on military tobacco use. Most participants thought tobacco should not be sold at military stores, but many also felt that this policy change was unlikely due to tobacco industry pressure, and DoD reliance on tobacco profits to support Morale, Welfare, and Recreation funds. Achieving a tobacco-free military will require changing pricing policy, but this study suggests that for effective implementation, military leadership must also understand and articulate more clearly the rationale for doing so. Previous work has found that adherence to military tobacco pricing policy is inconsistent at best. This study suggests that lack of knowledge about the policy and conflicting pressures resulting from the funding stream tobacco sales represent extend to high level military policy leaders. Without clearer information and direction, these leaders are unlikely to be able to establish and implement better tobacco pricing policy. © The Author 2016. Full text: This paper describes recent scientific and technological advances in the National Atmospheric Release Advisory Center (NARAC) that aid emergency management. The U.S. Department of Energy's NARAC system provides tools and services that help map the probable spread of hazardous material accidentally or intentionally released into the atmosphere. Located at Lawrence Livermore National Laboratory, NARAC is a national support and resource center for planning, real-time assessment and detailed studies of incidents involving a wide variety of hazards, including nuclear, radiological, chemical, or biological emissions. In recent years, the DOE National Nuclear Security Agency (NNSA) Office of Emergency Response and Chemical and Biological National Security Program (CBNP) have supported major upgrades and modernization of NARAC that have advanced the accuracy and utility of NARAC products for emergency planning and management. A new NARAC central modeling system, which became operational in the year 2000, has provided a higher-resolution suite of diagnostic and prognostic meteorological models, and a Lagrangian particle dispersion model, for producing predictions of air concentration, ground deposition, and dose. The 3-D meteorological data assimilation model, ADAPT, and Lagrangian particle dispersion model, LODI, allow the simulation of mean wind advection, turbulent diffusion, radioactive decay and production, bio-agent degradation, first-order chemical reactions, wet deposition, gravitational settling, dry deposition, and buoyant/momentum plume rise. The functions performed by this system have been fully automated to minimized response time for emergencies. An in-house version of the Naval Research Laboratory's COAMPS numerical weather prediction model is used to provide mesoscale forecasts. The fundamental policy choice of who has the right to do what to whom is a pivotal issue of governance. Over the last few decades, the answer to that question has become more restrictive to those who own and use natural resources as inputs into production processes. Increasingly, the beneficiaries of new policy initiatives are those who desire higher protection of groundwater quality. With respect to groundwater management, policy design increasingly reflects such diverse interests as agriculturists, industrialists, homeowners, local government officials and state officials. Policy design is becoming complex, in part because of this diversity and in part because scientific uncertainty hampers informed policy design. No umbrella federal legislation exists for managing groundwater resources. EPA's role has been mainly an advisory one on groundwater issues. The difficulties and responsibilities of protecting groundwater thus remain with the states. For the near future, it is the states that will address key policy choices with respect to groundwater quality management issues

The Atmospheric Release Advisory Capability (ARAC) project is a Department of Energy (DOE) sponsored real-time emergency response service available for use by both federal and state agencies in case of a potential or actual atmospheric release of nuclear material. The project, initiated in 1972, is currently evolving from the research and development phase to full operation. Plans are underway to expand the existing capability to continuous operation by 1984 and to establish a National ARAC Center (NARAC) by 1988. This report describes the ARAC system, its utilization during the past two years, and plans for its expansion during the next five to six years. An integral part of this expansion is due to a very important and crucial effort sponsored by the Defense Nuclear Agency to extend the ARAC service to approximately 45 Department of Defense (DOD) sites throughout the continental US over the next three years

The Alaska Science and Engineering Advisory Committee attempted to formulate a suitable state policy for global climate change. The main elements and rationale for this policy are described, along with lessons learned from the Montreal protocol on global ozone and the policy itself. A discussion of issues relating to public presentation and reaction to a climate change policy indicates that elements necessary for a strategy presenting a case for global change needs to be credible, simple, and unambiguous, with risks clearly defined. Society and business must see themselves as stakeholders in the issue, and policies must be formulated accordingly. The Montreal protocol provides an example of success in advanced planning on a major global issue. The six main components of the Alaskan policy relate to fossil fuel production and marketing, the economic mix of energy production for in-state use, the efficiency and effectiveness of energy end-use services, the impact of climatic change on Alaska as a geographic unit, Alaska as a high-latitude site for climate change monitoring and analysis, and Alaskan participation with other countries in research and policy development. 7 refs

Current urban information mechanisms in developing countries operate only through linear exchanges between institutions and users and therefore reinforce hierarchical relationships. Coupled with conflicting interests and perspectives of stakeholders in multilevel climate-governance and absence of grassroots information-networking for adaptation decision-making, there are therefore, existing information gaps. Central to urban sustainability is the need for citizen centered transparency (CCT) mechanisms that encompass and address the needs of the marginalized and vulnerable communities in developing countries especially. The study discloses the existing information gaps through information-needs assessment of stakeholders, and attempts to chart the desired course for responsible action within frame-work of Citizen Centered Transparency (CCT) mechanism. This involved analysis of several urban development projects for Indian metropolitans that mainly involved end-user association, and the parameters considered for breaking complexity for assessment included: a. Feedback: Ends-user feedback to improve resource consumption literacy and consequently urban behaviour and sustainable lifestyles(feedback technology, consumption displays, eco-labeling, billing, advisory services, sensor technology), and b. Administrative Traditions and Institutional Policy: Rewarding-punishing to enforce desired action(subsidies, taxation). The research thus answered: 1.Who gets the information whereas who requires it (Equity in Information Distribution)? and 2. How can information translate to responsible action in future (Transparency of Execution)? Findings suggested that, how, by using the CCT innovations it is practically possible to embed responsibilities in urban development planning, and manifesting environmental goals in municipal policies so that they bear clear potential short-term benefits, short-term costs, and have maximum compliance with the objectives of sustainable urban
