Resumen en español Desde una mirada crítica se construye una propuesta de incorporación de Internet como recurso pedagógico. El artículo gira en torno a tres ejes: primero, reconoce que la desigualdad social a través de la denominada brecha digital se ahonda más dentro del modelo económico neoliberal imperante. Planteamos que la solución se debe enfrentar en cuanto a reflexionar en torno a los usos de las tecnologías en los ámbitos educativos y no sólo en cuanto al acceso. En seg (mas) undo término, la gestión del conocimiento se presenta como un método creativo, innovador y propicio, al cual deben adscribirse las instituciones educativas. Por último, la conformación de comunidades de aprendizaje se propone como modalidad organizativa ideal para llevar a cabo la innovación Resumen en inglés From a critical view a proposal of incorporation of Internet as a pedagogical resource is constructed. The article centers around three axes: first, it recognizes that the social inequality through the denominated digital breach is more taken care of within the prevailing neoliberal economic model. We propose that the solution faces as far as reflecting around the uses of the technologies in the educational environment and not only access. Secondly, the management of the (mas) knowledge appears like a creative, innovating and propitious method, to which the education institutions must be assigned. Resumen en español Uno de los desafíos que debe enfrentar la sociedad cubana no solo es el envejecimiento de su población como un fenómeno objetivo y natural, sino el impacto de este problema demográfico en la llamada población económicamente activa, categoría que incluye a aquellas personas que están aptas para el trabajo en el rango de edad comprendido desde los 17 años hasta la edad de jubilación y se contempla en la ley No. 24 de la Asistencia y la Seguridad Social Cubana. Si (mas) estamos abogando por incrementar la productividad y la producción de los bienes materiales a nivel mundial, así como propender a las vías que coadyuven a que la especie humana tenga una existencia feliz, es contraproducente, entonces, el no poder contar con miles de millones de hombres, los cuales tienen como condición desplegar sus capacidades físicas y mentales en la creación de la riqueza colectiva. Por ello, se hace indispensable conocer las proyecciones que en cuanto a política social se realizan en nuestro país, en las cuales los trabajadores de la salud como parte de su rol social, constituyen uno de los agentes indispensables que garantizan que las expectativas en la esperanza de vida, entre otros beneficios, sean cada vez mayores. Resumen en inglés One of the challenges that Cuban society faces today is not only its population´s aging as an objective and natural phenomenon; but the impact of this demographic problem on the economically active population. This category includes persons with appropriate working capacity between 17 years and the retirement age, which is stated in the act No. 24 of Cuban Social Security. Since we support the idea of increasing production and productivity of goods worldwide, and we hold (mas) up any initiative to enhance human satisfaction or happiness, it is unprofitable for us not to be able to have millions of persons on condition that they spend their physical and mental capacity in the creation of our wealth. Thus, it is important to be aware of the new approaches regarding social policy, in which health workers have a central role to guarantee an increase in life expectancy, among other benefits.

Resumen en español Objetivo. Evaluamos la evolución de una población de 74 pacientes con diagnóstico de hemorragia subaracnoidea no traumática, en relación con la metodología de trabajo de un hospital público y la influencia de ciertos efectos operativos inherentes al sistema. Método. Los pacientes fueron internados en la Unidad de Terapia Intensiva (UTI) estudiados mediante tomografía axial computada (TC) y angiografía digital de 4 vasos, analizándose los tiempos de demora en la (mas) realización de los mismos. Consideramos la terapéutica al ingreso y el tratamiento quirúrgico (microcirugía o tratamiento endovascular). Evaluamos la morbimortalidad específica en función de la terapéutica implementada y la global de la población estudiada Resultados. De los pacientes sometidos a microcirugía (36) fallecieron 8 (22%) y de los casos que recibieron tratamiento endovascular (12) falleció un caso (8.33%). La mortalidad global de la población estudiada fue de 24 pacientes (32%) Conclusión. Consideramos que el diagnóstico y tratamiento de la HSA no traumática en el hospital público enfrenta dificultades operativas para la adecuada implementación de una atención neuroquirúrgica de excelencia en tiempo oportuno. Resumen en inglés Objective. We evaluated 74 patients with diagnosisnon traumatic subarachnoid hemorrhage in relation with the working methodology of a public hospital and its influence in certain operative effects inherent to the system. Method. Patients were treated in a neurosurgical intensive care unit, head CT scanning and cerebral angiography were performed; we analyzed the delay in their execution. We considered the initial therapy and the surgical treatment (microsurgery/ endovascu (mas) lar). We evaluated the specific morbimortality in relation to the implemented therapeutics and the global morbi- mortality of the population studied. Results. Of the patients that underwent microsurgical treatment (36) 8 died (22%) and 1 patient (8.33%) died of the 12 patients that underwent endovascular treatment. The overall mortality of the population studied was 24 patients (32%). Conclusion. We consider that the diagnosis and treatment of aneurysmal subarachnoid hemorrhage in a public hospital faces operative difficulties for the proper implementation of neurosurgical treatment of excellence in the appropriate time.

Resumen en inglés This paper outlines the evolution of the concept of continuing medical professional development in Uruguay, as that professional development has progressed from independent and varied efforts that were not coordinated or evaluated to a plan that is aimed at improving the quality of educational activities and programs and making them accessible to all the physicians in the country. In contrast to countries where scientific societies and medical associations preside over co (mas) ntinuing professional development, in Uruguay the Graduate School of the School of Medicine of the University of the Republic has taken on managing and developing this process. The key objective of this process is for the Graduate School to work in conjunction with other leading players in professional development to create a national system of continuing medical professional development. This system should be interinstitutional and include both public and private entities. By having the key public institutions (the Graduate School and the Ministry of Public Health) working with private entities (medical associations, scientific societies, and health care institutions), there should be an adequate balance of interests. The national professional development system should work in the most decentralized manner possible and should be based on a network of units distributed around the entire country, but with centralized coordination. The system's interinstitutional character should ensure high technical and ethical standards as well as a balance among the governmental, commercial, professional, and university components. This system should serve as the basis for extending this activity to all health personnel in Uruguay. While the national system is still being finalized, the Graduate School has implemented a process of accrediting institutions that are involved in continuing professional medical development. The aim of this accreditation process is to improve the educational offerings for physicians and to generate practical experience that will serve the future system.

Resumen en español La ciencia y la producción de saber científico están cambiando y esto muestra que la crisis de identidad de la ciencia contemporánea es una crisis de crecimiento de la que está surgiendo un modo nuevo de producción de conocimiento y tecnología. El nuevo saber es una tendencia hacia la superación de las barreras disciplinarias y el establecimiento de un cuadro del mundo nuevo, que reconozca su diversidad y complejidad intrínsecas. El pensamiento de la complejidad (mas) no niega el desarrollo alcanzado por el pensamiento simplificador prevalente en las ciencias. La humanidad se enfrenta actualmente a las consecuencias del daño a la naturaleza ocasionado por el hombre y el consecuente emerger y reemerger de problemas que afectan al mundo entero, como resultado de movimientos caóticos donde coexisten a la vez el orden/desorden, lo normal/lo patológico, endemia/pandemia/epidemia; emergen nuevas especies de patógenos bacterianos y virales como consecuencia de cambios ecológicos del medio ambiente, se incrementa la inestabilidad, la pobreza, el agotamiento de recursos naturales y el temor a desaparecer como especie y como planeta. En Cuba, el pensamiento de la complejidad, se ha dado a conocer desde la cátedra de la Complejidad del Instituto de Filosofía de la Academia de Ciencias; desde la salud, trabajan diferentes grupos a lo largo y ancho del país. Se pretende llevar a los lectores a reflexionar en la nueva forma de pensar que brindan estas nuevas teorías, útil para el estudio de cualquier problema de salud que afecte a los seres humanos en el mundo de hoy. Resumen en inglés Science and production of scientific knowledge are changing, which shows that the identity crisis of the contemporary science is a crisis in development from which a new mode of production of knowledge and technology emerges. The new knowledge is a tendency towards overcoming barriers among disciplines and setting up the picture of a new world that recognizes intrinsic diversity and complexity. The complexity thought does not deny the development reached by the simplifyin (mas) g thinking that prevail in sciences. The mankind faces at present the consequences of man-caused damage, and the resulting emergence and re-emergence of problems affecting the whole world, as a result of chaotic movements where order/disorder, normality/pathology; endemics/pandemics/epidemics coexist; new bacterial and viral pathogenic species emerge due to environmental changes; instability, poverty, depletion of natural resources and fear to dissapearance of the species and the planet as such are on the rise. In Cuba, the Complexity Chair of the Philosophy Institute of the Academy of Sciences has made the thought of complexity known whereas in the field of healthcare, different groups are working on this issue throughout the country. This paper intends that readers make reflections on the new way of thinking offered by these novel theories, which are really useful for studying any health problem that may have an impact on the today's world population.
