Sertralina: Eficacia y tolerabilidad como tratamiento antidepresivo coadyuvante en pacientes con dolor crónico/ Effectiveness and tolerability as coadjuvant antidepressant treatment in patients with chronic pain

Resumen en español Introducción: Nos planteamos, en este estudio, valorar la eficacia de la sertralina como antidepresivo y su tolerabilidad, mediante el control de los efectos secundarios relacionables con posibles interacciones farmacológicas, cuando se utiliza asociada a otros fármacos analgésicos y coadyuvantes en pacientes con síndromes dolorosos crónicos de diferentes etiologías. Método: Estudiamos a 34 pacientes (28 mujeres y 6 hombres), edad media de 59,3 ± 15,9 años (mas) (rango 22-78). La procedencia fue intrahospitalaria en 3 ocasiones y extrahospitalaria en 31. Con dolor de origen osteoarticular en 15 casos, neuropático 11, mixto 6 y vascular en 2. Causa degenerativa 15 pacientes, traumática 7, tumoral 3, esencial 2, isquémica 1 y otras en 6. Dolor generalizado en 10 casos, regional en 19 y localizado en 5. Evolución superior a 12 meses en 25, de 6 a 12 meses en 3 y menos de 6 meses en 6 pacientes. Intensidad del componente depresivo asociado valorado, antes y después del tratamiento con sertralina, con test de Hamilton. Dosis utilizadas. Duración del tratamiento. Fármacos asociados. Efectos secundarios. Seguimiento. Resultados: Encontramos valores de intensidad de la depresión, pretratamiento, que corresponden en 13 pacientes a depresión grave y en 21 a moderada. Postratamiento, medida realizada en 27 pacientes, valores de corte normal en 23 pacientes y patológico en 4. La dosis diaria media inicial y final de sertralina ha sido de 26,4 ± 5,9 y 50,7 ± 4,2 mg; el tiempo medio de tratamiento de 10,5 ± 4,7 meses. Se empleó tratamiento asociado con Aine en el 82,3% de los casos, opiáceo débil en el 64,7%, opiáceo potente en el 26,4%, anticonvulsivante 61,7%, corticoides el 2,9% e hipnótico el 5,8%. En ningún caso encontramos efectos secundarios relacionados con la administración de sertralina. La mejoría del ánimo se valoró de discreta en 4 pacientes, buena en 18 y muy buena en 5. Continuan en tratamiento 18 pacientes, 6 han sido dados de alta, 6 están perdidos, 1 ha fallecido y en 3 se suspendió, por ineficacia 1 caso y por elección del paciente en 2. Conclusiones: Comprobamos, en estos pacientes, una excelente tolerabilidad de la sertralina cuando se utiliza asociada a otros fármacos analgésicos y coadyuvantes. Destaca el valor terapéutico de este fármaco en los casos de depresión secundaria asociada en algunos síndromes dolorosos. Resumen en inglés Introduction: The aim of this study was to determine the effectiveness of sertraline as antidepressant and its tolerability through the control of side effects attributable to potential drug interactions, when used in combination with other analgesic and coadjuvant drugs in patients with chronic pain syndromes of different etiologies. Method: We studied 34 patients (28 women and 6 men) with an average age of 59.3 ± 15.9 years (range 22-78). Their referral had an in (mas) patient origin in 3 cases and an outpatient origin in 31. Pain etiology was osteoarticular in 15 cases, neuropathic in 11, mixed in 6 and vascular in 2. Its cause was degenerative in 15 patients, traumatic in 7, carcinogenic in 3, essential in 2, ischemic in 1 and some other cause in 6. The severity of associated depression was assessed before and after the treatment with sertraline, using the Hamilton test. Dosage used. Length of treatment. Associated drugs. Side effects. Follow-up. Results: Depression severity scores obtained before the treatment suggested severe depression in 13 patients and moderate depression in 21. After the treatment, depression scores were obtained in 27 patients, with normal values in 23 patients and abnormal in 4. Initial and final average daily dose of sertraline was 26.4 ± 5.9 y 50.7 ± 4.2 mg, respectively, and the average length of treatment was 10.5 ± 4.7 months. Adjuvant therapy with NSAI was used in 82.3% of the patients, weak opiate in 64.7%, powerful opiate in 26.4%, anticonvulsant in 61.7%, corticoid in 2.9% and hypnotic in 5.8%. No side effects associated to the administration of sertraline were found in any patient. Mood improvement was described as moderate in 4 patients, satisfactory in 18 and very satisfactory in 5. Eighteen patients still continue with the treatment, 6 have been discharged, 6 have been lost, 1 has died and 3 have discontinued the treatment because of lack of effectiveness in 1 case and patient election in 2. Conclusions: Sertraline has shown an excellent tolerability in these patients when used in combination with other analgesic and coadjuvant drugs. This drug has a particularly high therapeutic value in cases of secondary depression associated to several pain syndromes.

Resumen en español Objetivo: Aplicar las técnicas de cultivo in vitro de queratinocitos así como los principios de la ingeniería tisular al epitelio urinario humano, con el fin de reconstruir tridimensionalmente una mucosa vesical humana in vitro, apta para trasplantar. Material y Métodos: Se obtuvieron muestras de mucosa vesical de pacientes programados para cirugía abierta de próstata, previo consentimiento de los mismos, las cuales fueron cultivadas in vitro, para proceder posterio (mas) rmente al estudio histomorfológico de los tejidos obtenidos. Resultados: Se obtuvo un tejido tridimensional compuesto por una submucosa bioartificial a base de un gel de fibrina y fibroblastos, sobre la que descansan las células uroepiteliales, pudiendo utilizar una malla de ácido poliglicólico, que facilite la manipulación de la mucosa y el posterior injerto de la misma. El tejido obtenido tenía el aspecto de un epitelio inmaduro con muy escasa reacción a citoqueratinas, sin poderse demostrar inmunohistoquímicamente el desarrollo de una membrana basal. Conclusiones: Las técnicas de cultivo in vitro de queratinocitos son aplicables a otros epitelios, entre ellos el urotelio humano. En un periodo de tiempo relativamente corto se puede obtener un tejido in vitro tridimensional apto para trasplantar, precisando posiblemente de las interacciones con el lecho receptor para poder madurar. Resumen en inglés Objetive: The purpose of this study is to apply the in vitro keratinocyte culture techniques and the tissue engineering principles to human urothelium, to reconstruct an in vitro three-dimensional human bladder mucosa, suitable for grafting. Material and Methods: Biopsy specimens of human bladder mucosa were obtained from patients undergoing suprapubic prostatectomy, in vitro cultured and finally, an immunohistochemical study was made. Results: A three-dimensional in vitr (mas) o tissue was obtained, composed of a bio-artificial submucosa (fibrin gel and fibroblast) where the uroepithelial cells were seeding. We used a biodegradable polyglycolic acid mesh to facilitate the tissue manipulation and implantation. An immature epithelium was obtained with a weak immunostaining to cytokeratins. The immunohistochemical study could not demonstrate the development of basement membrane. Conclusions: In vitro keratinocyte culture techniques could be applied to other epithelial tissues like the urothelium. We obtained a three-dimensional in vitro tissue suitable for grafting in a relatively short time, which needs the matrix interactions in order to mature.

Resumen en portugués Com o objetivo de esclarecer se o mecanismo acromático é independente do cromático ou, pelo contrário, existem interações entre ambos, realizamos dois experimentos. No primeiro, estudamos a profundidade, induzida pela cromoestereopsia de pares de tiras isoluminantes e anisocromáticas inseridas em fundo acromático, que variou em luminância em relação às partes cromáticas. No segundo experimento, utilizamos quatro pares de tiras cromáticas inseridas em fundo d (mas) e grade de onda quadrada. Nestes fundos, as listras podiam ser isso ou anisoluminantes e isso ou anisocromáticas. Os resultados demonstraram que: 1) a profundidade percebida variava em função da variação da luminância, mudando o sinal da profundidade quando se invertia o sinal da diferença em luminância entre fundo e figura; 2) é possível a cromoestereopsia em baixa luminância; 3) evidenciou-se uma fraca interação entre a variação de contraste cromático e a de contraste de luminância; 4) a interação destes dois mecanismos deve ocorrer em nível pré-retiniano; 5) as respostas dos dois mecanismos são separáveis. Resumen en inglés In order to know if the mechanisms that process luminance are independent of those that process colour, or, conversely, if there is an interaction between them, we have executed two experiments. In the first, we studied how the changes in the perception of depth elicited by chromostereopsis with couples of isoluminants and anisochromatic patches inserted in a uniform background, which varied in luminance from trial to trial. In a second experiment, we used a square-wave g (mas) rating background, in which every strip changes the luminance of the colour. The different background could be, on the one hand, isoluminant/anisoluminant, or, on the other hand, isochromatic/anisochromatic. The results showed: 1) that perceived depth varied as a function of the luminance variation. That is, reversing the depth when the sign of the difference in luminance between figure and background changes; 2) that it is possible to elicit chromoesteropsis at isoluminance; 3) a weak interaction between the variation of chromatic contrast and achromatic contrast; 4) that interactions between the two mechanisms should occur at pre-retinal levels; and 5) the responses of the two mechanisms are separable.

Resumen en español OBJETIVOS: Identificar, comparar y analizar las redes sociales de dos grupos de adultos mayores pobres, beneficiarios de dos tipos de viviendas sociales, en términos de tipos de nodos, frecuencia y motivo del vínculo, antes y después del traslado a sus nuevos domicilios. MÉTODO: Se realizó un estudio descriptivo de beneficiarios de viviendas en edificio, o bien de viviendas en condominios. El universo en estudio fue el colectivo de personas asignatarias a viviendas e (mas) n edificio (n = 152) y a viviendas en condominio (n = 124), entre 1998 y 2001, a las cuales se les aplicó una encuesta en su domicilio. RESULTADOS: La mayoría de los participantes en el estudio informaron que la asignación de la vivienda implicó para ellos mudarse a barrios distantes de su antiguo domicilio, lo que provocó, entre otras consecuencias, un cambio en sus redes sociales. Ambos grupos tienen, en general, redes sociales similares, constituyendo los hijos el principal nodo de apoyo, antes y después del cambio de domicilio. Después del traslado se redujeron los vínculos en general, excepto en el caso de los adultos de vivienda en edificio, quienes incrementaron levemente sus vínculos con los hijos y en forma destacada con personas con quienes tienen vínculos débiles y que no son propiamente familiares ni amigos. Los vínculos con las instituciones también disminuyeron, aunque en el caso de las instituciones de adultos mayores aumentaron, para los residentes en condominios de 28% a 65%, y para los asignatarios de edificios, de 31% a 45%. CONCLUSIONES: Se sugiere que en los programas semejantes al estudiado se tome en cuenta la importancia que revisten la conservación y el fortalecimiento de las redes sociales creadas antes del traslado, y que se fomente la generación de vínculos en el nuevo sitio de residencia. Resumen en inglés OBJECTIVES: To identify, compare, and analyze the social networks of two groups of poor, older adults in two types of public housing projects in Chile, in terms of the types of relationships and the frequency and purpose of their interactions, before and after moving to their new homes. METHODS: A descriptive study was conducted of public housing beneficiaries in apartment buildings versus condominiums. The study pool was the group of people assigned to housing in buildin (mas) gs (n = 152) and condominiums (n = 124) from 1998 to 2001. A questionnaire was administered in their homes. RESULTS: Most study participants indicated that the move to public housing meant leaving their old neighborhood for a new one, a change that, among other consequences, produced changes in their social networks. In general, both groups have similar social networks, with their children being the strongest source of support, both before and after the move. After the move, social interaction generally decreased, except among the adults living in apartment buildings, for whom it increased slightly with family members, and in a superficial manner, with others who had weak connections and were neither friends nor family. Interactions with establishments also decreased, although interactions with senior services increased for the condo residents from 28% to 65% and for apartment residents from 31% to 45%. CONCLUSIONS: It is recommended that similar programs strive to preserve and strengthen the social networks in place prior to moving and encourage the formation of relationships in the new residences.

Bloqueos nerviosos periféricos de la extremidad inferior para analgesia postoperatoria y tratamiento del dolor crónico/ Lower limb continuous peripheral nerve blocks for postoperative analgesia and chronic pain

Resumen en español Existe un interés creciente por la realización de los bloqueos de nervio periférico (BNP) debido a sus potenciales beneficios como los concernientes a las interacciones de los fármacos anticoagulantes y los bloqueos neuroaxiales. Los BNP de la extremidad inferior, y sobre todo, los bloqueos periféricos del nervio ciático son el pariente pobre de las técnicas de anestesia regional y, en general, son poco conocidos y por tanto poco utilizados. En este artículo se re (mas) aliza una revisión de los bloqueos del plexo lumbosacro, realizando especial énfasis en los bloqueos continuos mediante catéteres para analgesia postoperatoria y para el tratamiento del dolor crónico. La utilización de anestésicos locales de larga duración de acción, asociada a un escaso bloqueo motor, como es el caso de la ropivacaína, nos permite combinar técnicas de punción única para conseguir una adecuada analgesia intraoperatoria, con las técnicas de perfusión continua para analgesia postoperatoria. Es necesario un conocimiento anatómico preciso, así como de los territorios cutáneos de inervación de las ramas del plexo lumbosacro, para la realización de estas técnicas de bloqueo. La introducción de diferentes técnicas de imagen, fundamentalmente la ultrasonografía, para la localización de las estructuras nerviosas, facilita la realización de estos bloqueos y disminuye el riesgo de lesiones de los órganos adyacentes. La realización de los bloqueos continuos de nervio periférico ofrece el beneficio de una analgesia postoperatoria prolongada, con menores efectos adversos, mayor grado de satisfacción del paciente, y una recuperación funcional más rápida después de la cirugía. Resumen en inglés There is increasing interest in peripheral nerve blocks (PNB) because of potential benefits relative to interactions of anticoagulants and central neuraxial techniques. Among all the regional anesthesia procedures, PNB of the lower limb, and specially sciatic nerve block, are the less known and rarely used. In this paper, we summarize the recent advances in lumbosacral plexus block, with special remark on continuous catheter blocks for postoperative analgesia and chronic (mas) pain treatment. The use of long action local anes-thetics combined with a weak motor block, like ropivacaine, permits the combination of a single skin injection technique to achieve an adequate intraoperative analgesia with continuous infusion ones for postoperative analgesia. A detailed knowledge of anatomy as well as cutaneous nerve distribution is needed for the performance of these techniques. The introduction of several image-based techniques, principally ultrasound imaging, may improve the accuracy of nerve localization and needle placement. It also can help to prevent accidental puncture to vessels and other neighbouring structures. Continuous plexus and peripheral nerve blocks offer the potential benefits of prolonged analgesia with fewer side effects, greater patient satisfaction, and faster functional recovery after surgery.

Resumen en español A partir de la presentación de un par de ceremonias del culto de jurema, este trabajo se vuelca en el análisis de ciertas cuestiones del campo religioso brasileño. Los aspectos destacados son, fundamentalmente, la dinámica entre agencias religiosas y los sectores eruditos que se interesan en el estudio de las religiones subalternas, las redes de agencias - su densidad, los diversos trayectos que permiten los vínculos entre agentes de diversa legitimidad etc. -, la b� (mas) �squeda de un valor operacional para "sincretismo", la homogeneización del campo, es decir, la absorsión de cultos de lógicas débiles por otros de lógicas fuertes. Resumen en inglés Taking two Jurema ritual ceremonies as the starting point, this study concentrates on the analysis of some issues regarding the Brazilian religious universe. The main aspects brought into focus are, basically, the interactions between religious agencies and scholarly sectors interested in the study of marginal, secondary religions; the agencies networks - their cohesiveness, the various crossing paths they allow, the bonds between agents of different legitimacy etc.; the (mas) search for an operational value for syncretism; the homogenization of the universe, namely, the assimilation of rituals based on weak logical systems by rituals based on strong logical systems.
