Resumen en español Se presenta una revisión actual, puesta al día, y punto de vista de los autores sobre un tema sumamente novedoso y atractivo, como es la Cirugía Endoscópica Transluminal por Orificios Naturales (NOTES: Natural Orifice Translumenal Endoscopic Surgery). La mayoría de los trabajos revisados se han realizado en animales de experimentación, pero la publicación de la colecistectomía por vía transvaginal, y la aparición de editoriales y artículos de revisión sobre el (mas) tema, nos llevan a realizar una serie de preguntas no resueltas actualmente sobre este tipo de cirugía, que representa un avance potencial para conseguir "una cirugía endoscópica sin cicatrices, sin infecciones, con mínimos requerimientos de anestesia y una inmediata recuperación". Resumen en inglés A current review and update of an exceedingly novel and appealing topic, namely natural orifice transluminal endoscopic surgery (NOTES), is discussed, as well as the authors' viewpoint thereon. Most reviewed studies were performed in laboratory animals, but reports on transvaginal cholecystectomy and the emergence of editorials and review articles on this topic pose a number of as yet unanswered questions on this type of surgery, which represents a potential advance towar (mas) ds "endoscopic surgery with no scars, no infection, minimal anesthesia requirements, and immediate recovery".

Cirugía sin ingreso de pacientes con fístulas arteriovenosas para hemodiálisis: Actividad integrada en un servicio de cirugía general/ Outpatient surgeries of patients with arteriovenous fistulas for hemodialysis: Integrated activity in a general surgery unit

Resumen en español Introducción: El aumento de pacientes que precisan tratamiento renal sustitutivo, sobre todo en el grupo de pacientes sometidos a hemodiálisis, supone un reto en incremento de actividad y de ocupación de recursos para los servicios de cirugía. Las complicaciones relacionadas con los accesos vasculares son la causa fundamental de ingresos en muchas unidades de diálisis. La cirugía sin ingreso puede disminuir la ocupación de camas hospitalarias, reduce la lista de es (mas) pera y las complicaciones relacionadas con un ingreso innecesario. Material y métodos: Hemos realizado un estudio prospectivo de las intervenciones realizadas en el período 1998-2009 para la creación o la reparación de fístulas arteriovenosas (FAV) para hemodiálisis, con el objetivo de conocer el nivel de ambulatorización, resultados, complicaciones y su posible impacto en la tasa de ingresos de los pacientes en hemodiálisis. La actividad fue realizada dentro del funcionamiento global del servicio de cirugía general sin unidad específica de cirugía mayor ambulatoria (CMA). Las intervenciones las realizaron varios cirujanos del servicio interesados en el tema, pero sin dedicación exclusiva a éste (su actividad es la de cualquier cirujano general) y sin guardias específicas. La cirugía ambulatoria se organizó dentro de la actividad ordinaria del servicio de cirugía general sin una unidad específica, ni cirujanos especialmente dedicados a la misma. Resultados: Desde la apertura de nuestro hospital en 1998 hasta diciembre de 2009 hemos realizado un total de 2.413 intervenciones en 1.229 pacientes (primeros accesos y reparaciones de los mismos). La cirugía programada supuso el 74,8% de las intervenciones; el 25,2% restante fueron intervenciones urgentes. El porcentaje global cirugía ambulatoria fue del 82% (89% en cirugía programada y 60% en cirugía urgente). Se produjeron un 6% de ingresos imprevistos. No hubo mortalidad postoperatoria. El número de ingresos fue de 0,09 episodios por paciente año con una estancia media de 0,2 días por paciente y año. Conclusiones: La mayoría de las intervenciones relacionadas con las FAV, incluso la cirugía urgente, se pueden realizar en régimen ambulatorio dentro de la actividad habitual de un servicio de cirugía. Se evitan así costes asociados con la ocupación de camas hospitalarias y se disminuyen las complicaciones relacionadas con el ingreso. Resumen en inglés Introduction: The increase of prevalent haemodialysis patients is a challenge for surgery units. Vascular access related complications are the main cause of hospital admissions in many dialysis units. Outpatient surgery could decrease waiting lists, cost and complications associated to vascular access. Material and methods: We have performed a prospective study of the vascular access related surgery in a ten year period. Outpatient surgery was included with the rest of th (mas) e activity in a general surgery unit and was performed by not exclusive dedicated surgeons. Results: Since 1998 to December 2009 we performed 2,413 surgical interventions for creating and repairing arteriovenous fistula in 1,229 patients, including elective and emergency surgery (74.8% and 25.2% respectively). Outpatient procedures were performed in 82% of cases (89% in elective and 60% in emergency surgery). There were unexpected admissions secondary to surgical complications in 6% of patients. There was no postoperative mortality. The rate of admissions was 0.09 events and 0.2 days per patient/year. Conclusions: Outpatient surgery is possible in a high percentage of patients to perform or to repair an arteriovenous fistula, including emergency surgery. Vascular access surgery can be included in ordinary activity of a surgical unit. Outpatient vascular access surgery decreases unnecessary hospital admissions, reduces costs and nosocomial complications.

Resumen en español La cirugía abierta capaz de hacer grandes exéresis y reparaciones en tejidos frágiles, está siendo paulatinamente sustituida durante esta última década por la laparoscópica, por sus enormes beneficios para el paciente, sin olvidar las dificultades para su aprendizaje por los cirujanos que tienen que suplir estas deficiencias con mayor esfuerzo y estrés. La cirugía robótica viene a suplir las limitaciones de la cirugía laparoscópica convencional haciendo cirug� (mas) �as más ergonómicas y más precisas, sobre todo para aquellas cirugías más complejas y difíciles de acceder como es la prostatectomía radical. En esta revisión se hace un recordatorio de historia y aplicaciones clínicas de nuevas tecnologías avanzadas y robotizadas y en comparación con la cirugía abierta y la laparoscopia convencional. Resumen en inglés Over the last decade, open surgery, which is able to perform large extirpations and repairs of fragile tissues, is gradually being substituted with laparoscopic surgery due to the high benefits the latter entails for the patients, an also due to the learning difficulties for surgeons who must make up for such deficiencies applying higher efforts and a larger amount of stress. Robotic surgery stands in for the limitations of conventional laparoscopic surgery by means of pe (mas) rforming more ergonomic and more accurate surgeries, particularly in the case of the most complex and difficult to access operations, such as radical prostatectomy. This review will perform a reminder of the history and clinical applications of new advanced and robotic technologies, and also a comparison with open surgery and conventional laparoscopy.

Cirugía periapical en dientes posteriores maxilares: Revisión de la bibliografía/ Periapical surgery of maxillary posterior teeth: A review of the literature

Resumen en español En los últimos años, la cirugía periapical ha evolucionado gracias a la incorporación de avances diagnósticos y técnicos. El objetivo del presente artículo es realizar una revisión bibliográfica de los trabajos publicados sobre cirugía periapical en los dientes antrales; hemos revisado el Medline y las revistas españolas de Odontología desde 1974 hasta el 2003. Comentamos la anatomía del seno maxilar, el diagnóstico de las lesiones periapicales y la relació (mas) n del seno maxilar con los dientes antrales; también la técnica quirúrgica, sus consideraciones especiales, y el pronóstico de la cirugía periapical en estos dientes. Los trabajos recientes plantean que la proximidad de los dientes antrales al seno maxilar, no es una contraindicación para la cirugía periapical, y recomiendan su realización en dientes con patología periapical crónica, refractarios al tratamiento endodóncico convencional, a pesar de la proximidad del seno maxilar. Resumen en inglés In recent years, periapical surgery has evolved thanks to new diagnostic and technical advances. A review is made of the literature on periapical surgery of the antral teeth, based on a Medline search and on the revision of Spanish dental journals in the period between 1974 and 2003. The anatomy of the maxillary sinus is discussed, along with the diagnosis of periapical lesions and the relation of the maxillary sinus to the antral teeth. The surgical technique, special co (mas) nsiderations and prognosis of periapical surgery in these teeth are also addressed. Recent studies postulate that the proximity of the antral teeth to the maxillary sinus should not be viewed as a contraindication to periapical surgery, and recommend such surgery in teeth with chronic periapical disease that are refractory to conventional endodontic management, despite the proximity of the maxillary sinus.

Resumen en español Presentamos los resultados iniciales del programa de cirugía laparoscópica retroperitoneal del Hospital Universitario La Paz desde julio de 2004 hasta diciembre de 2004. El programa se inició tras 2 años de experiencia en cirugía laparoscópica pélvica. La experiencia inicial ha sido lo suficientemente buena como para reducir progresivamente las contraindicaciones y aumentar el número de cirugías realizadas con dicha técnica. Resumen en inglés We present the initial results of retroperitoneal laparoscopic surgery in the Hospital Universitario La Paz from july 2004 to december 2004. The program started after 2 years of pelvic laparoscopy surgery practice. The initial experience has been so good that we have reduced the contraindications for this technique and we have increased the number of laparoscopic surgery cases.

Resumen en español El primer informe impreso sobre una cirugía de tumor intracerebral en el mundo es el realizado por Bennett y Godle, en 1884, en Londres, sobre un glioma. Se publican, en la misma época, intervenciones sobre este tipo de cirugía en Francia, con Chipault, en Italia con Durante, en Estados Unidos con Keen, en Alemania con Krause y Oppenheim, entre otros. En México Rafael Lavista realiza la primera cirugía sobre una neoplasia intracerebral en 1891, y se publica en 1892. (mas) También se hace referencia, en esta serie informada por Lavista, al inicio de la cirugía de la epilepsia en México. El autor no ha encontrado, para el siglo XIX, otro informe de intervención sobre neoplasias intracerebrales en el ámbito hispánico. Resumen en inglés The first rapport of intra-cerebral tumor surgery was provided by Bennett & Goodle, in London, 1884. Worldwide this kind of surgery was performed in France by Chipault, in Italy by Durante, in the United States by Keen and in Deutchland by Krause & Oppenheim.Lavista in Mexico City operated on intra-cerebral tumor in 1891, and the rapport was printed in 1892. In the same publication, Lavista exhibited the first cases of epilepsy surgery. Since now, it is the first rapport of this kind of surgery in the Spanish spoken world.

Resumen en español Caso clínico: Paciente intervenido hacía 7 años de LASIK que desarrolla una anisometropía cilíndrica secundaria a cirugía de desprendimiento de retina (DR) en OD. Presenta diplopia con gafas (OD -1,25 -2,75 x 60º AV=1,0; OI +0,25 AV=1,2). Se adaptó una lente de contacto (LC) ajustada a su topografía corneal eliminando la diplopia. Discusión: La cirugía del DR puede provocar cambios refractivos, induciendo anisometropía que puede dificultar su corrección con g (mas) afas. El uso de una LC adaptada a la geometría corneal disminuyó los síntomas anisometrópicos permitiendo la corrección refractiva. Resumen en inglés Case report: A male with cylindrical anisometropia secondary to retinal detachment (RD) surgery in the right eye (OD) was referred for contact lens (CL) fitting. His refraction was OD -1.25 -2.75 x 60º VA 1.0 and OS +0.25 VA 1.2. He was complaining of diplopia with spectacles. Seven years prior to the RD surgery, he had undergone LASIK without complications. The diplopia was eliminated after a CL was fitted according to his corneal topography. Discussion: RD surgery can (mas) cause anisometropic refractive changes. In patients with diplopia and asthenopia, spectacles are not well tolerated. CL fitting according to post-LASIK corneal geometry succeeded in refractive correction with less anisometropic symptoms.

Resumen en español Caso clínico: Se describen los hallazgos oculares debidos a un déficit de vitamina A en un varón de 50 años. Fue sometido a una cirugía de bypass intestinal dos años antes. Después de tratamiento con vitamina A oral, los síntomas mejoraron. Discusión: La incidencia de obesidad mórbida sigue aumentando alarmantemente en todo el mundo, siendo actualmente el tratamiento mas adoptado en la mayoría de los pacientes la cirugía de bypass intestinal. Este tipo de ciru (mas) gía puede provocar un déficit de vitamina A, con importantes consecuencias oftalmológicas, que podrían incluso llegar a la ceguera sin un correcto tratamiento. Por consiguiente, se debería considerar seriamente esta patología hasta ahora poco frecuente en países desarrollados. Resumen en inglés Case report: We describe ocular findings due to a vitamin A deficiency in a 50-year-old man. The patient had undergone intestinal bypass surgery two years before. After therapy with oral vitamin A the symptoms improved. Discussion: The incidence of morbid obesity is increasing throughout much of the developed world, with intestinal bypass surgery the treatment of choice for most people with the condition. This type of surgery can lead to a vitamin A deficiency, with remar (mas) kable ophthalmological consequences, which without correct treatment may ultimately cause blindness. For this reason, the pathology, even though rare at present, must considered seriously.

Resumen en español Caso clínico: Descripción de una mujer operada de estrabismo con anestesia tópica, con conjuntivitis exudativa en el postoperatorio inmediato, que a los 15 días tuvo un síndrome de efusión uveal. El tratamiento fue la exploración quirúrgica, retirada de las suturas de vycril, antibioterapia y corticoterapia tópica, subtenoniana y sistémica, aislándose Staphylococcus epidermidis en las muestras intraoperatorias. Evolucionó favorablemente en 4 meses. Discusión: (mas) La efusión uveal secundaria a cirugía de estrabismo no está publicada en la literatura revisada. La infección de las suturas en la cirugía escleral de retina por Staphylococcus epidermidis se considera una posible, aunque rara, causa. Resumen en inglés Case report: A woman was operated on for strabismus under topical anesthesia. An exudative conjunctivitis was diagnosed immediately after the surgery, and a uveal effusion syndrome diagnosed 15 days later. Surgery exploration, vycril suture removal and therapy with systemic, sub-tenon, and topical corticosteroid and antibiotics was initiated. Intraoperative culture revealed evidence of Staphylococcus epidermidis. Good resolution occurred in 4 months. Discussion: Uveal eff (mas) usion secondary to strabismus surgery has not been reported in the published literature. Implant and suture infection by Staphylococcus epidermidis following scleral buckling surgery may be a possible, but rare, cause of the uveal effusion syndrome.

Resumen en español Caso clínico: Se describe un brote de Síndrome Tóxico del Segmento Anterior después de cirugía vítrea. Dos pacientes habían sido intervenidos exclusivamente mediante vitrectomía mientras que los otros tres fueron sometidos a vitrectomía y algún otro procedimiento del segmento anterior. Discusión: El Síndrome Tóxico del Segmento Anterior es una inflamación postoperatoria estéril provocada por alguna sustancia no infecciosa que alcanza el polo anterior durant (mas) e la cirugía. Suele ocurrir en brotes y aunque la mayoría de los casos han sido descritos después de cirugía del segmento anterior, los casos que presentamos demuestran que es posible su aparición tras cirugía vítrea. Resumen en inglés Case report: An outbreak of Toxic Anterior Segment Syndrome after vitreoretinal surgery is reported. Two patients underwent exclusively vitrectomy while the other three patients were operated of vitrectomy and some other anterior segment procedure. Discussion: Toxic Anterior Segment Syndrome is a sterile postoperative inflammation due to any non infectious substance that reaches the anterior segment during surgery. It occurs in outbreaks and while most of the cases have b (mas) een reported after anterior segment procedures, this case demonstrates that development after vitreoretinal surgery is also a possibility.

Resumen en español La cirugía y la medicina no evolucionaron en forma paralela, con discrepancias, reservas, belicosidad, desprecios e incluso estudios universitarios separados durante bastante tiempo. La Cofradía de San Cosme, fundada en Francia en 1260, para supervigilar el ejercicio profesional quirúrgico de los barberos (cirujanos-barberos de toga corta), tuvo la oposición de la Facultad de Medicina de París. Los intereses contrapuestos de la universidad, Cofradía y Barberos, que (mas) persistieron hasta fines del siglo XVIII, perjudicaron el progreso de la cirugía. En la primera mitad del siglo XIX, el avance de la cirugía continuó enfrentándose al dolor, la hemorragia y a la infección. El control de ésta debió pasar por la antisepsia, asepsia y finalmente por la aparición de las sustancias antimicrobianas, sulfamidados y antibióticos, que permitirían a los cirujanos abordar y solucionar problemas de mayor relevancia de la especialidad, curativos y preventivos Resumen en inglés Surgery and medicine have not evolved in parallel. There have been discrepancies, bellicosity, contempt and even separate university studies during a long time. The Saint Cosme Brotherhood, founded to supervise the professional practice of barbers (short robe surgeon-barbers) in France in 1260, was opposed by the Faculty of Medicine in Paris. The conflicting interests of the university, Brotherhood and Barbers, that persisted until the 18 th century, impaired the progress (mas) of surgery. In the first half of the 19 th century, the advance of surgery continued facing pain, hemorrhage and infection. Resumen en español Las fístulas ureterales se relacionan con la cirugía ginecológica, cirugía digestiva y cirugía urológica reconstructiva del aparato urinario superior. Están descritas fístulas ureterovaginales, ureteroduodenales, ureterocolónicas, ureteropleurales, ureterovasculares, etc. Sin embargo, las fístulas ureterocutáneas del muñón ureteral tras nefrectomía son una entidad muy inusual. Presentamos dos casos así como su resolución mediante cirugía. Resumen en inglés The ureteral fistulas are related to the gynecological surgery, digestive surgery and reconstructive urologic surgery of the upper urinary tract. Fistulas are described ureterovaginal, ureteroduodenal, ureterocolonic, ureteropleural, ureterovascular, etc. However, the ureterocutaneous fistulas of the ureteral stump after nephrectomy are a very unusual entity. We report two cases as well as their resolution by means of surgery.

Los términos Mesh fueron "Prostatectomy", "Prostatic neoplasm (mas) ", "Transuretral resection prostate", "Impotence" y los términos libres fueron "erectile dysfunction" y "prostatectomy". Los estudios incluían pacientes con cáncer de próstata intervenidos para la extirpación de la glándula mediante cirugía abierta (retropúbica), laparoscópica o robótica. Resultados: Se seleccionaron diez estudios observacionales de calidad moderada y 29 series de casos de baja calidad. Los estudios observacionales presentaban los menores porcentajes de disfunción eréctil en pacientes intervenidos mediante cirugía robótica (3-51%). La PR retropúbica (36-91%) y la laparoscópica registraban cifras superiores de aparición de impotencia. Los estudios que comparaban intervenciones, cirugía y radioterapia obtenían con las radiaciones menores índices de disfunción sexual (3-72%). En las series de casos, los menores porcentajes de disfunción eréctil acontecieron en pacientes con cirugía robótica (22%), seguido de cirugía laparoscópica (40%) y de PR retropúbica (41,4%). Conclusiones: Los resultados, aunque deben interpretarse con cautela dada la limitada calidad de los estudios, presentan efectos similares en la evaluación de las diferentes técnicas quirúrgicas, observándose que la cirugía robótica registra los menores porcentajes en relación con la aparición de impotencia sexual. Resumen en inglés Objective: To assess erectile dysfunction in patients with prostate cancer undergoing surgery by radical prostatectomy, laparoscopic prostatectomy or robotic prostatectomy. Material and methods: Systematic Review of literature based on a search strategy (2000-10) in MedLine, Embase, Cochrane Library, CRD, ECRI, and Hayes. Mesh terms used were "Prostatectomy", "Prostatic Neoplasm", "Transuretral Resection Prostate", "Impotence" and as free terms "erectile dysfunction" and (mas) "prostatectomy". Studies included patients with prostate cancer underwent by prostatectomy radical with open surgery (retropubic), laparoscopic or robotic surgery. Results: Ten observational studies with moderate quality and 29 case series with low quality were selected. Observational studies showed lower percentages of erectile dysfunction after intervention in the patients underwent robotic surgery (3-51%). Radical surgery (36-91%) and laparoscopic surgery showed higher values of impotence. In the studies that compared surgery versus radiotherapy, the results were better for radiotherapy (3-72% erectile dysfunction). In the case series, lower percentages of erectile dysfunction were shown in patients underwent to robotic surgery (22%), the following was for laparoscopic surgery (40%) and open radical prostatectomy (41.4%). Conclusions: This result should be considered with caution because of the low methodological quality of the studies included. However, the different surgical techniques assessed showed similar effects in the two types of studies included and we found that robotic surgery presented lower percentages of sexual impotence.

Resumen en español Desde el año 2007 se están realizando distintos procedimientos en cirugía laparoendoscópica por puerto único (laparoendoscopic single-site surgery), incluyendo nefrectomía, pieloplastia, adenomectomía prostática y, con el perfeccionamiento del instrumental laparoscópico, prostatectomía radical. Presentamos nuestra experiencia inicial en prostatectomía radical laparoscópica utilizando el dispositivo Single-Incision Laparoscopic Surgery Port® de Covidien y 2 tr (mas) ocares auxiliares de 5mm colocados lateralmente para triangulación. El Single-Incision Laparoscopic Surgery Port® permite una inserción precisa y sencilla a través de una incisión de Hadson. El puerto flexible contiene 3 cánulas de 5mm o 2 cánulas de 5mm y una de 12mm para facilitar el intercambio de instrumental a través de la incisión única. Este abordaje disminuye la morbilidad por sangrado, hernias y/o lesión de los órganos internos y mejora los resultados cosméticos. El puerto único forma parte del desarrollo natural de la cirugía mínimamente invasiva. Se necesita más experiencia para determinar los beneficios intra y postoperatorios de la cirugía laparoendoscópica por puerto único en comparación con la laparoscopia convencional. Resumen en inglés Since 2007, various urological procedures have been performed with laparoendoscopic single-site surgery (LESS surgery), including nephrectomy, pyeloplasty, simple prostatectomy and, with the refinement of laparoscopic instrumentation, radical prostatectomy. This paper reports our initial experience in radical prostatectomy using the SILS TM Port from Covidiem and two lateral 5-mm trocars for triangulation. The SILS TM Port allows for accurate, simple insertion through a H (mas) adson incision. The flexible port accomodates three 5-mm cannulas or two 5-mm cannulas and a 12-mm port for easier instrument exchange through a single incision. This approach decreases morbidity from bleeding, hernia and/or internal organ damage and improves cosmetic results. One-port single-incision laparoscopy is part of the natural development of minimally invasive surgery. Future research is required to assess the intraoperative and postoperative benefits of LESS surgery as compared to standard laparoscopy.

Resumen en inglés Ten percent of cirrhotic patients with severely compromised liver function will require a surgical procedure. This article reviews the surgical risk associated with liver damage and surgery. The most important prognostic factor is the degree of functional impairment of the liver. It is evaluated using the Child-Pugh score, which has limitations that have been partially overcome by the Model for End Stage Liver Disease (MELD) score. Cardiac surgery has the highest risk, wh (mas) ile extra-thoracic and extra-abdominal procedures have the lowest risk. The mortality for abdominal surgery fluctuates between 11 and 76%. Biliary surgery is associated with frequent complications and mortality, which seem to decrease when the procedures are laparoscopic. There are few series that evaluate risk in Child C patients. In liver resective surgery, liver function impairment and magnitude of the excision determine the risk. A high serum creatinine, cardiac failure and emergency surgery are independent risk factors. Although MELD score is useful to predict surgical risk, decision-making must be based on an individualized evaluation of each patient and careful planning of surgical procedures

Resumen en inglés Open and endovascular surgery are therapeutic alternatives for the treatment of abdominal aortic aneurism. The development of guidelines for its treatment requires a thorough analysis of available evidence to recommend the best treatment for each country's reality. Prospective randomized trials have shown best initial results with endovascular surgery, with higher hospital costs than open surgery. The requirement of anatomical suitability for the placement of endovascular (mas) prostheses limits the universal use of endovascular surgery. Moreover, this type of surgery needs a strict imaging and clinical follow up due to the high rates of late complications, which range from 20% to 40%. Many of these complications require further surgical interventions, elevating costs of treatment. The initial benefit of endovascular surgery is lost during long follow up as survival curves become similar to those of open surgery. Even for patients with a high surgical risk, the benefits of endovascular surgery are doubtful
