Valor de la cuantificación de la alfafetoproteína sérica en embarazos gemelares/ Value of the quantization of the serumal alphafetoprotein in gemellary pregnancy

Resumen en español Con el objetivo de valorar si existe relación entre los resultados adversos en los embarazos gemelares, fundamentalmente parto pretérmino, recién nacido de bajo peso y muertes fetales, con los valores séricos de la alfafetoproteína durante el segundo trimestre del embarazo, se realizó una investigación en el Hospital Ginecobstétrico «Ana Betancourt de Mora» de la provincia de Camagüey, Cuba. Para la ejecución de la misma se llevó a cabo un estudio analítico (mas) transversal con 38 embarazos gemelares cuyos partos se produjeron durante el año 2003, a través de la revisión del libro de partos de ese año y el de documentación de entrada de muestras de sangre para cuantificación de la alfafetoproteína. El 47 % de los embarazos cursaron con aumento de la alfafetoproteína. La mayoría de los partos se produjeron con 37 semanas ò más (N=26), sin embargo, la mayoría de este grupo tenían valores normales de la proteína, mientras que entre los casos de partos con menos de 37 semanas, el 75 % cursó con elevación de la proteína. 40 recién nacidos bajo peso y 36 normopeso fueron los productos de los 38 embarazos. Entre los bajo peso predominaron aquellos cuyos embarazos cursaron con alfafetoproteína elevada y entre los normopeso y los de un feto con peso menor de 2500g y el otro con 2500g ó más, predominaron aquellos con alfafetoproteína normal. Se presentaron ocho muertes fetales, siete de ellas con alfafetoproteína elevada. Las elevaciones de la alfafetoproteína tanto para los partos pretérmino como para los recién nacidos bajo peso oscilaron entre 2 y 3.4 múltiplos de la mediana para determinada edad gestacional. La evaluación de la alfafetoproteína en los embarazos gemelares es de gran valor ya que permite identificar las gestaciones de mayor riesgo, y brindarle una mejor atención a las mismas para así evitar resultados no deseados Resumen en inglés With the objective to value if there is relation among the adverse results in gemellary pregnancies, fundamentally preterm delivery, low birth weight and fetal deaths, with the serumal values of the alphafetoprotein during the second trimester of pregnancy, an investigation at «Ana Betancourt de Mora» Gynecobstetric Hospital of Camagüey province, Cuba was conducted. A cross-sectional analytic study with 38 gemellary pregnancies was carried out whose labors were produce (mas) d during the year 2003, through the review of the labor book of that year and the one of blood samples entrance documentation for the quantization of the alphafetoprotein. The 47% of pregnancies progressed with an increase of the alphafetoprotein. The majority of labors were produced with 37 weeks or more (N=26), nevertheless, the majority of this group had protein normal values, while among the labor cases less than 37 weeks, the 75% progressed with protein elevation. Forty low birth weight and 36 normoweight were the products of 38 pregnancies. Among of the low weight dominated those whose pregnancies progressed with elevated alphafetoprotein and between the normoweight and of the one fetus with weight less than 2500g and the other with 2500g or more, dominated those with normal alphafetoprotein. Eight fetal deaths were presented, seven of them with high alphafetoprotein. The alphafetoprotein elevations so much for the preterm labors as for the low birth weight oscillated between 2 and 3,4 M.O.M. fundamentally. The alphafetoprotein evaluation in gemellary pregnancies is a great value which permits to identify the gestations of greater risk, and to offer a better care to avoid not desire results

La técnica es confiable y exacta para la separación y cuantificación de hemoglobina A2 en individuos portadores de hemoglobina AC, Cb talasemia, hemoglobina AS y hemoglobina SC, lo cual es en muchos casos de interés clínico. Resumen en español La oseointegración de un implante dental está condicionada por el tipo de material del implante, su topografía superficial y el tipo de recubrimiento. Aunque son varios los materiales utilizados para la fabricación de implantes dentales, actualmente el titanio es el material de preferencia por a su comportamiento inerte al entrar en contacto con los tejidos biológicos. Como la topografía superficial influye en la adhesión y proliferación celular, los implantes den (mas) tales son sometidos a tratamientos que crean irregularidades superficiales en la escala micro y sub-micrométrica. Así mismo, recubrimientos basados en minerales de origen orgánico son utilizados para mejorar la deposición de hueso sobre el implante. El objetivo de este estudio es realizar el cultivo de células osteoblásticas sobre sustratos de Ti-6Al-4V con cuatro tipos de superficies típicamente usadas en implantes dentales para determinar cuál de ellas tiene mejor comportamiento en términos de adhesión celular, proliferación celular y biomineralización. Las células osteoblásticas fueron cultivadas durante 24 horas para el ensayo de adhesión y durante 1, 2, 5, 6 y 7 días para el ensayo de proliferación. La biomineralización fue evaluada caracterizando mediante stereo zoom y SEM los depósitos de mineral coloreados mediante la técnica de tinción con alizarina roja. La cuantificación de estos depósitos de mineralización se realizó utilizando una herramienta de procesamiento de imágenes. Los resultados obtenidos revelan que las superficies rugosas y con recubrimiento poseen mejor comportamiento. Ninguna de las superficies estudiadas tuvo carácter tóxico para las células y todas presentaron perfiles de adhesión celular, proliferación celular y biomineralización. Resumen en inglés The bone integration of dental implants is dependent of type of material of implant, its superficial topography and also by the type of coating. Although there are many the materials used for the manufacture of dental implants, currently the titanium is the choice material by its inert behavior when it contact with biological tissues. As the superficial topography influenced on cellular adhesion and proliferation, the dental implants underwent treatments creating superfic (mas) ial irregularities in the micro and sub-micrometric scale. Likewise, the coats based on organic-origin minerals are used to improve the bone deposition on implant. The aim of present paper is to carry out the osteoblasts cells culture on Ti-6AI-4V substrates using four different types surfaces typically used in dental implants to determine which of them have the better behavior in terms of cellular adhesion, cellular proliferation and biomineralization. The osteoblasts cells were cultivated during 24 hr for the adhesion assay and during 1, 2, 5, 6 and 7 days for the proliferation assay. The biomineralization was assessed by characterization with estereozoom and SEM of the mineral depots colored using dying technique with red alizarin. The quantization of these mineralization depots was performed using a tool of images processing. The results obtained showed that the rough surfaces and coated have a better behavior. None of the study surfaces had a toxic character for cells and all showed cellular adhesion profiles, cellular proliferation and biomineralization.

Resumen en español Introducción: El ácido láctico es considerado como un indicador temprano de supervivencia y/o mortalidad en pacientes que necesitan procedimientos quirúrgicos cardiovasculares. Objetivo: Determinar el comportamiento del ácido láctico en sangre arterial durante la cirugía cardiaca pediátrica y el pronóstico de dichos pacientes al egresar de la unidad de cuidados intensivos. Material y Método: Se realizó un estudio prospectivo, analítico y observacional de 55 pa (mas) cientes pediátricos que recibieron circulación extracorpórea en el Cardiocentro Pediátrico "William Soler". Se tomaron muestras de sangre arterial una vez realizada la inducción anestésica (ácido láctico inicial), precirculación extracorpórea y al final de ella. Se consideró hiperlactatemia cuando el valor de acido láctico en sangre arterial estuvo por encima de 3 mmol/L durante el intraoperatorio y postoperatorio inmediato. Resultados: La cuantificación del ácido láctico en diferentes momentos del proceder quirúrgico fue significativa para el valor al inicio del intraoperatorio y el valor en circulación extracorpórea, no lo fue así para la relación de éste último con el valor al final de la misma. Se demostró que el tipo de cardiopatía no estuvo asociada con el estado al egreso. Conclusiones: la lactatemia perioperatoria constituye un indicador temprano de supervivencia en estos pacientes. Los valores de lactato sérico durante la circulación extracorpórea fueron mayores cuando se emplea ringer lactato en las soluciones de cebado. El tipo de cardiopatía no constituye un factor predictor de supervivencia. Resumen en inglés Introduction: Lactic acid is considered as an early indicator of survival and/or mortality in patients with cardiovascular surgical procedures. Objectives: To determine behavior of lactic acid in arterial blood during pediatric cardiac surgery, and the prognosis of such patients when are discharged of intensive care unit. We took samples of arterial blood at the end of anesthesia induction (initial lactic acid), extracorporeal pre-circulation, and at the end of it. We considered the hyperlactacidemia when the lactic acid value in arterial blood was higher 3 mmol/L during the immediate intra- and postoperative period. Results: Lactic acid quantization in different times of surgical procedure was significant for the initial value of intraoperative period, and that of the extracorporeal circulation, but not for its relation with the value at the end of it. It was possible to demonstrate that the type of cardiopathy was not associated with the discharge status. Conclusions: Perioperative lactacidemia is an early indicator of survival in these patients. Serum lactate values during extracorporeal circulation were higher when it is used lactated ringer in primer solutions. The type of cardiopathy is not a predictor factor of survival.

Resumen en español INTRODUCCIÓN: se requiere una técnica de análisis específica que permita dar seguimiento al estudio de la estabilidad intrínseca del follaje seco pulverizado de Parthenium hysterophorus L. (escoba amarga), para la obtención de una forma farmacéutica de utilidad antiparasitaria con los requisitos de calidad, seguridad y eficacia exigidos. OBJETIVO: demostrar la especificidad de la técnica analítica de cromatografía líquida de alta eficacia para la cuantificació (mas) n de partenina en el polvo de P. hysterophorus para su aplicación en estudios de estabilidad. MÉTODOS: se aplicó cromatografía líquida de alta eficacia a muestras degradadas de P. hysterophorus, bajo condiciones degradativas en medio ácido, básico y oxidativo. Se evaluó la especificidad de la técnica de análisis para detectar el componente de interés sin interferencias de sus productos de degradación y su posible utilidad en estudios de estabilidad del polvo de la planta. RESULTADOS: la cromatografía líquida de alta eficacia para cuantificar partenina en el polvo de P. hysterophorus resultó específica en las condiciones de trabajo establecidas y puede emplearse en estudios de estabilidad del sólido en el polvo de la planta. CONCLUSIONES: la técnica de cromatografía líquida de alta eficacia propuesta es específica y se recomienda su utilización en los estudios de estabilidad del sólido en polvo de la planta. Resumen en inglés INTRODUCTION: it is required a specific analysis technique allowing the follow-up to stability study intrinsic of Parthenium hysterophorus L. (escoba amarga) powdered dry foliage to achieve in a pharmaceutical way a antiparasitic usefulness with the quality, safety and effectiveness demanded requirements. OBJECTIVE: to demonstrate the specificity of analytical technique of high-performance liquid chromatography for quantization of partenine in the powder of P. hysterophor (mas) us for its application in stability studies. METHODS: high performance liquid chromatography was applied to P. hysterophorus degraded samples under degradation conditions in an oxidative, basic and acid medium.The analysis technique specificity was assessed to detect the interest component without interferences of its degradation products and its possible usefulness in studies on solid stability in the plant powder. RESULTS: high-performance liquid chromatography to quantify the presence of partenine in the P. hysterophorus powder was specific in established work conditions and may be used in solid stability studies of plant powder. CONCLUSIONS: the high-performance proposed is specific and it is recommended in solid stability studies of solids in plant powder.
