Resumen en español Objetivos: Evaluar la calidad de los sitios web de centros y organizaciones sobre temas farmacoeconómicos de los países de la Unión Europea, Estados Unidos y Canadá, mediante un sistema validado con criterios explícitos. Métodos: Identificación en el web de instituciones, centros relacionados con farmacoeconomía y economía de la salud en el ámbito de los 24 países de la OCDE desde el año 1999, a través de buscadores mediante palabras clave predeterminadas. Di (mas) seño y validación de un cuestionario de calidad de la información sanitaria contenida en Internet según las normas establecidas en la bibliografía. Evaluación de la calidad de los centros de farmacoeconomía y economía de la salud a través de este cuestionario. Resultados: En 23 de los 26 ítems que contiene el cuestionario validado, la Q de Cochran figura como estadísticamente significativa. El coeficiente de fiabilidad obtenido en el cuestionario fue de 0,90 y el valor obtenido en la correlación de Pearson fue de 0,812. Los 33 centros de farmacoeconomía y economía de la salud evaluados pueden dividirse en 3 subgrupos de acuerdo con las puntuaciones obtenidas en el cuestionario: centros de alta calidad, calidad media, subgrupo al que pertenece la mayoría de los centros, y calidad baja. Los centros que se corresponden con el subgrupo de alta calidad son: Leonard Davis Institute of Health Economics, Centre for Health Economics, Agency for Healthcare Research & Quality, Health Economics Research Unit, The Institute of Health Economics y Health Economics Resource Center. Conclusiones: Los criterios incluidos en el cuestionario que son los más utilizados en la evaluación de la calidad de páginas web sanitarias son: credibilidad, contenido, descripción, vínculos, diseño, interactividad y salvaguarda. La calidad de sitios web de centros relacionados con farmacoeconomía y economía de la salud analizados en este trabajo mediante los criterios establecidos en el cuestionario varía desde un 30,4% hasta un 79,8%. Es necesario un sistema validado, con criterios explícitos, para evaluar la calidad de la información sanitaria en Internet. The 33 pharmacoeconomics and health economics centers evaluated were divided into three subgroups according to the scores obtained on the questionnaire: centers of high quality, average quality (the majority), and low quality. The centers in the high-quality subgroup were: the Leonard Davis Institute of Health Economics, the Centre for Health Economics, the Agency for Healthcare Research and Quality, the Health Economics Research Unit, the Institute of Health Economics and the Health Economics Resource Center. Conclusions: The criteria included in the questionnaire used to evaluate the quality of websites on health were: credibility, content, description, links, design, interactivity and safeguards. The quality of the websites of centers related to pharmacoeconomics and health economics analyzed in this study using the criteria established in the questionnaire varied from 30.4% to 79.8%. Resumen en inglés Background:Coronary angioplasty was introduced in Chile in 1982, but only after the introduction of stents it became commonplace. However, the results of this procedure at the national level remain largely unknown. Aim: To assess the results and characteristics of coronary angioplasty and develop a national registry in Chile (RENAC). Patients and Methods: All centers performing angioplasty were invited to contribute prospectively with the clinical, angiographic, procedura (mas) l characteristics and results of all consecutive coronary angioplasty procedures attempted between June 2001 and October 2002. Results: In 10 centers, 1768 lesions were treated in 1484 procedures (22.98% in women). Mean age was 60.8±11.3 years old. Diabetes was found in 21.8%, hypertension in 60.2%, dyslipidemia in 52.0%, active smoking in 40.0%, renal failure in 6.2%. Myocardial infarction was recent in 28.4% and acute in 19.7%. Forty eight percent of patients had unstable angina and 15.5% had stable angina. Fifty three percent of patients had single vessel disease and 71%, normal left ventricular eyection fraction. One vessel was treated in 90.6% of patients and 81.7% of lesions were treated with stents, 17.9% only with baloon and in 0.4% with atherectomy. Angiographic success was obtained in 95.2%. Clinical success was obtained in 92.2%, and 95.1% in patients without acute myocardial infarction. Overall inhospital death was 2.2%. In patients without myocardial infarction, the figure was 1.1%. Conclusions: Coronary angioplasty in Chile is performed mostly for the treatment of acute coronary syndromes. Stents are the most frequently used devices. The high success, low mortality and complications observed are comparable to North American registries (Rev Méd Chile 2004; 132: 913-22)

Resumen en inglés Obesity is a chronic disease with an increasing prevalence in all groups of age, and is associated to increased general mortality and cardiovascular risk. The multidisciplinary non surgical approach must be the treatment of choice for obese subjects. However, the results of such approach among subjects with severe or morbid obesity, are unsatisfactory. In this group of patients, bariatric surgery and specifically gastric bypass achieves good long term results, maintaining (mas) a low body mass index, reducing complications and improving quality of life. Considering the widespread practice of bariatric surgery in Chile, the Nutrition Unit of the Ministry of Health formed a task force to propose update guidelines for the surgical treatment of obesity. These guidelines were proposed after a thorough literature review and discussion with groups that practice bariatric surgery using defined protocols. This document provides a summarized version of the guidelines. The first section discusses the background for bariatric surgery and the second part give specific recommendations for patient management and the formation of reference centers for surgical management of obesity

Resumen en inglés Background: There is little information about the prevalence of asthma in Latin American children. Aim: To determine the prevalence of asthma among Chilean school age children. Subjects and methods: The prevalence of respiratory symptoms related to asthma in schoolchildren aged 7 and 13 years, coming from South Santiago, Central Santiago, Valdivia and Punta Arenas, was determined using the methodology of the International Study on Asthma and Allergies in Childhood (ISMC). (mas) A random sample of school children aged 6-7 and 13-14 years from each locality was selected. This resulted in 24)470 surveyed children (11,723 aged 6-7 years and 12,747 aged 13-14 years). Results: The current prevalence of wheezing ("wheezing in the last 12 months") ranged from 16.5% to 20.ó% in children aged 67 years, and from 6.8% to 11.7% in children aged 13-14 years. The cumulative prevalence of asthma ("asthma ever"), ranged from 9.7% and 16.5% in the 6-7 years group and from 7.3% to 12.4% in those aged 13-14 years. Conclusions: This study found much higher figures for prevalence of respiratory symptoms related to asthma in school children than those previously reported in this country, with a significant variability between centers (p<0.05). The prevalence of asthma in Chilean schoolchildren is as high and variable as that reported in industrialized countries

Resumen en inglés The nutritional impact of CONIN nutritional recovery centers must be evaluated, considering the current epidemiological situation in Chile and the new therapeutic focus giving more emphasis to ambulatory treatment. Aim: To analyze the nutritional status of children treated at traditional CONIN centers, the reason for their admission and the factors associated with changes of weight for age index during the hospitalization. Patients and Methods: During the year 2000, the r (mas) ecords of 561 patients discharged from the traditional CONIN centers throughout the country were retrospectively analyzed. The changes in weight and height during admission and the possible factors influencing these changes, were determined. Results: The average lapse of stay was 3.9 months; 78% of children had concomitant diseases at admission and 18.7% required to be admitted in a general hospital. One third was admitted with normal weight or even overweight according to the weight for age index, and 31.1% was undernourished. Resumen en español El síndrome de MEN2A es una enfermedad autosómica dominante que se caracteriza por el desarrollo de cáncer medular de tiroides, feocromocitoma e hiperplasia de paratiroides. Mutaciones en el ret proto-oncogén se asocian con MEN2A, con una penetrancia cercana al 100%. El gen se encuentra en el cromosoma 10q11.2 y codifica para una proteína transmembrana con función de receptor del tipo tirosina quinasa. Mutaciones que afectan el dominio extracelular de la proteína e (mas) stimulan la dimerización espontánea del receptor y un aumento de la actividad de tirosina quinasa basal. El codón 634 codifica para una cisteína, y es considerado un sitio hot-spot por encontrarse mutado en el 85% de las familias con MEN2A. Para este sitio, nuestro grupo desarrolló en 2002 una metodología de detección indirecta y económica. Ante una familia sospechada de MEN2A, se aplicó esta estrategia, que reveló un codón 634 sano. Por posterior secuenciación se confirmó que el paciente índice portaba una mutación en el codón 611. Se desarrolló una nueva estrategia familia-específica por PCR mutagénica, que permitió diagnosticar en nuestro país a todos los integrantes de la familia con costos accesibles. Un niño en el cual se halló la mutación, fue tiroidectomizado preventivamente, y a la fecha goza de buena salud. De esta manera, combinando la estrategia de detección de mutaciones en el sitio hot-spot y un posterior diseño de otra metodología familia-específica se pudo diagnosticar e intervenir preventivamente a la familia, sin enviar todas las muestras al extranjero. Resumen en inglés MEN2A is an autosomic dominant disease, characterized by medullary thyroid cancer, pheochromocytoma and parathyroid hyperplasia. Mutations in the ret proto-oncogene are associated with this disease, with almost 100% of pennetrance. The gene, situated on chromosome 10q11.2, codes for a transmembrane protein with a tirosinkinase-like receptor function. Mutations that affect its extracellular domain, stimulate spontaneous homodimerization and elevate the basal tirosinkinase (mas) activity. The codon 634 of the gene is considered a hot-spot site, since it is mutated in 85% of the MEN2A families. Our group developed in 2002 an indirect and costless strategy to detect alterations in this site. We present a family suspected of having MEN2A. We applied our PCR based indirect strategy on the DNA of the index patient and found that there was no mutation in that site. Posterior sequencing of exon 10 and 11 confirmed that the mutation affecting this family was in codon 611. Thus, we developed a new costless family-specific strategy based on mutagenic PCR and enzymatic cuts to diagnose all the family members. A seven-year old boy with this mutation was preventively thyroidectomized. In this way, combining the indirect methodology for codon 634 previously developed by our group, and a posterior family-specific mutation detection strategy, we were able to diagnose and intervene presymptomaticly the family members, avoiding sending all the samples to foreign centers.

Resumen en inglés Coronary angiography and percutaneous treatment of stenotic lesions have expanded in the last few years, due to availability of better diagnostic equipment. The femoral technique applied to this aims has prevailed, considering its efficacy, safety and wide acceptance. Since the beginning of this decade, an alternative access has been developed, in relation to miniaturization of the required elements to perform coronary diagnostic and therapeutic procedures. This new radia (mas) l artery access is supported by multiple reports from many centers around the world that are increasingly using the technique. With this access it is possible to perform all the regular procedures done regularly through the femoral route. This case report illustrates a coronary angiography study through the radial access, followed by a stent implantation, through the same route
