Resumen en español Paraguay, país mediterráneo ubicado en el corazón de America del Sur, con una superficie de 406.542 Km2 y con una población de 5,8 millones de habitantes importa toda la sal que consume de países cercanos como la Argentina, Brasil y Chile. En el presente estudio observacional, de tipo descriptivo utiliza el método ecográfico para determinar el tamaño y las características de la glándula tiroides, se examinaron 1034 escolares de ambos sexos de 13 distritos del pa (mas) ís y fue realizado durante 3 meses del año 2000. Los lugares elegidos tenían antecedentes de bajo consumo de sal yodada con valores de yoduria por debajo de 50 µg/L, como también la medición de la yoduria por el método de trinitrito , el tenor de yodo en sal y la relación talla/peso de los escolares estudiados. El 82.9% del volumen tiroideo de los escolares resultó dentro de los limites normales. Se detectó un volumen tiroideo aumentado en el 17.1% de los niños según la edad y sexo y en 22% según la superficie corporal. Nódulos tiroideos se observaron en 5 niños y sólo1 niño tenía antecedentes de haber tenido cirugía tiroidea. El 53% de los distritos presentaron medias urinarias de yodo por encima de 200 µg/dL con un promedio de 301.3 µg/dL. El 40% de los distritos presentaron yodurias óptimas (entre 100 y 199 µg/dlL) con una media de 145 µg/dL. Solo el 7% presentó una media urinaria de yodo bajo 100 µg/dL. El estudio demostró una mejoría en la yodación de la sal de consumo humano al compararlo con los realizados en 1988 en el Paraguay,sin embargo,existen riesgos de producir en la población tirotoxicosis y enfermedades tiroideas autoinmunes por el porcentaje de yodo en la sal en aumento, por lo que se recomienda un seguimiento y control con los ajustes necesarios de los niveles de yodación Resumen en inglés Paraguay, a mediterranean country located in the heart of South America , with a surface of 406.542 Km2 and a population of 5,8 million inhabitants, import all the salt to consume from neighbord countries as Argentina, Brazil and Chile. The present observational study, of descriptive type uses the ultrasonography method to determine the size and the characteristics of the thyroid gland in 1,034 school children of both sexes of 13 districts of the country. The elected places had antecedents of under consumption of iodized salt with urinary iodine excretion values below 50 µg/L, as well as the mensuration of the yoduria for the trinitrito method, the level of iodine in salt and the relation height/weight of the studied children. Thyroid volume of school children was in 82.9% of cases within the normal limit. Increased thyroid gland volume was observed in 17.1% of the children by age and sex and 22% by body surface. Fifty three porcent of the districts presented a median of urinary iodine excretion above 200 µg/dL with an average of 301.25 µg/dL. Forty porcent of the districts presented a normal urinary iodine excretion between 100 and 199 µg/dl with a median of 145 µg/dL. Only 7% presented a median urinary iodine excretion below 100 µg/dL. This study showed an improvement of iodination of salt for human consumption when comparing with those carried out in 1988 in Paraguay, nevertheles, still there are some risks in the population of thyrotoxicosis and thyroid autoinmunes diseases due to an excesive intake of iodine in the salt. For this reason, a pursuit and control with the necessary adjustments of iodine salt levels could be recommended

Resumen en español Objetivo: La corrección del déficit de yodo disminuye en la incidencia de bocio nodular tóxico. El fin de este estudio es determinar la etiología de la tirotoxicosis en dos áreas de Galicia con diferente prevalencia de bocio, 15 años después de la campaña institucional para el uso de sal yodada. Los resultados actuales fueron comparados con los de un estudio similar realizado en las mismas áreas hace 10 años. Métodos: Se incluyeron 202 pacientes con tirotoxicos (mas) is atendidos en La Coruña (costa) y Lugo (interior) desde enero de 2000 a mayo de 2002. Se les realizó historia clínica y exploración física, y se determinaron hormonas tiroideas, anticuerpos antitiroideos e inmunoglobulina estimuladora de tiroides. Se realizó también gammagrafía tiroidea con 99m tecnecio o 123yodo. Resultados: En el grupo completo el 58,6% de los casos fueron diagnosticados de bocio nodular tóxico; el 30,3%, de enfermedad de Graves; el 7.1%, de tirotoxicosis inducida por yodo y el 3,5% de tiroiditis subaguda. En la zona costera los porcentajes fueron 52,2, 37,0, 5,4 y 4,3%, respectivamente. En la zona de interior el 64,2% fueron diagnosticados de bocio nodular tóxico; el 24,5%, de Enfermedad de Graves; el 8,5%, de tirotoxicosis por yodo y el 2,8% de tiroiditis agudas. En todos los grupos el diagnóstico más frecuente fue bocio nodular tóxico. Conclusiones: En determinadas áreas de Galicia el bocio nodular tóxico continúa siendo la causa más frecuente de tirotoxicosis. Es necesario insistir en la aplicación y seguimiento de los programas de suplementación con yodo. Resumen en inglés Objective: Correction of iodine deficiency diminishes the incidence of toxic nodular goitre. The aim of this study was to assess the etiology of thyrotoxicosis in two areas with different goitre prevalence in Galicia, fifteen years after the institutional campaign of salt iodination. Results of the present survey are compared with those from a study performed ten years ago. Methods: Two hundred and two thyrotoxic patients attended in La Coruña (coastal zone) and Lugo (in (mas) land zone) from January 2000 to May 2002 were included. Clinical and exploratory data were recorded. Thyroid hormones, thyroid-directed antibodies and thyroid stimulating immunoglobulin (TSI) were measured. A thyroid 99m technetium or 123 iodine scintigram was performed. Results: In the whole group 58.6% of the cases were diagnosed of nodular goitre, 30.3% of Graves' disease, 7.1% of iodine induced thyrotoxicosis and 3.5% of subacute thyroiditis. In coastal zone these percentages were 52.2, 37.0, 5.4 and 4.3%, respectively. In the inland area, 64.2% nodular goitre, 24.5% Graves' disease, 8.5% iodine induced thyrotoxicosis and 2.8% subacute thyroiditis. The most frequent diagnosis in both the whole group and in each area was toxic multinodular goitre. Conclusions: In some areas of Spain, toxic nodular goitre continues to be the most frequent cause of thyrotoxicosis. It is necessary to insist on implementation and monitoring of iodine supplementation programs.

Resumen en inglés Background: In Chile, the recommendation for salt iodination was reduced from 100 to 40 ppm, after detecting high urinary iodine excretion in school age children. Aim: To assess iodine nutrition in school age children after the modification of salt iodination. Subjects and methods: Iodine nutrition was studied in 3712 school age Chilean children of primary level in 4 areas (Calama, Santiago, Temuco and Punta Arenas) through palpation of thyroid gland, urinary iodine excre (mas) tion and iodine concentration in salt for human consumption of the surveyed areas. Results: A 6.4% goiter prevalence was found (5,9% in boys and 6,8% in girls). Grade Ia goiter was more common (5.0%), without detecting differences between areas (7.3% in Calama, 5.9% in Santiago, 5.9 in Temuco and 6.7 in Punta Arenas). Iodine concentration in salt for human consumption of the areas was adequate according to the recent modification of Chilean legislation (25.4 mgI/g salt in Calama, 36.2 in Santiago, 27.7 in Temuco and 33.4 in Punta Arenas). Urinary iodine excretion was very high according to recommendation in boys and girls (2838 and 2852 mgI/g salt in Calama, 700 and 837 in Santiago, 1318 and 1438 in Temuco, 407 and 312 in Punta Arenas). Conclusions: Endemic goiter is not longer a problem in Chile. Salt iodination is in agreement with present legislation. However, urinary iodine excretion increased in Calama and Temuco, did not change in Santiago and decreased only in Punta Arenas in spite of the changes in the legislation. It is necessary to investigate the causes of the high urinary iodine excretions detected in school age children, and it is very important to have a continuous surveillance of iodine nutrition in Chile (Rev Méd Chile 2003; 131: 1390-8)
