  1. Thyroidal radioisotope uptake in euthyroid cats : a comparison between 131I and 99MTcO4

    Full Text Available Two thyroidal evaluation systems in euthyroid cats (n = 12 were compared. A single, confirmed hyperthyroid cat was included for interest. Firstly, thyroidal uptake of an intravenous bolus of approximately 111 MBq (3 mCi 99MTcO4- was estimated by using a scintillation camera and calculating the ratio of thyroid to salivary activities at 20 min and 4 h. Thyroid to salivary activity ratios were 1:1 at 20 min and 2:1 at 4 h. Two discrete areas of salivary uptake were identified, namely a parotid/mandibular complex and a more rostral buccal/sublingual complex. These results were compared to radioiodine uptake of an oral dose of approximately 0.925 MBq (25 mCi 131I using a standard thyroid uptake system, measured at 1, 2, 4, 6, 8, 10, 12, 24 and 48 h after administration. Mean radioiodine thyroidal uptake started at 33 % at 1 h, stabilised at 21 % between 4 and 24 h, and dropped to 18 % at 48 h. There was a significant correlation between the early thyroid:salivary ratio of the parotid/mandibular complex and the radioiodine uptake at 12 h.

  2. Intraarticular and intravenous administration of 99MTc-HMPAO-labeled human mesenchymal stem cells (99MTC-AH-MSCS): In vivo imaging and biodistribution

    Introduction: Therapeutic application of intravenous administered (IV) human bone marrow-derived mesenchymal stem cells (ahMSCs) appears to have as main drawback the massive retention of cells in the lung parenchyma, questioning the suitability of this via of administration. Intraarticular administration (IAR) could be considered as an alternative route for therapy in degenerative and traumatic joint lesions. Our work is outlined as a comparative study of biodistribution of 99m Tc-ahMSCs after IV and IAR administration, via scintigraphic study in an animal model. Methods: Isolated primary culture of adult human mesenchymal stem cells was labeled with 99m Tc-HMPAO for scintigraphic study of in vivo distribution after intravenous and intra-articular (knee) administration in rabbits. Results: IV administration of radiolabeled ahMSCs showed the bulk of radioactivity in the lung parenchyma while IAR images showed activity mainly in the injected cavity and complete absence of uptake in pulmonary bed. Conclusions: Our study shows that IAR administration overcomes the limitations of IV injection, in particular, those related to cells destruction in the lung parenchyma. After IAR administration, cells remain within the joint cavity, as expected given its size and adhesion properties. Advances in knowledge: Intra-articular administration of adult human mesenchymal stem cells could be a suitable route for therapeutic effect in joint lesions. Implications for patient care: Local administration of adult human mesenchymal stem cells could improve their therapeutic effects, minimizing side effects in patients.

  3. Radio localization of inflammatory bowel diseases using 99mtc-colloidal bismuth subcitrate

    A correct diagnosis and adequate assessment of disease activity is crucial in the management and surveillance of inflammatory bowel diseases (IBDs). However, in certain occasions the obtained results by means of traditional techniques are not correlated specifically with the inflammatory state of the mucosal wall, mainly in case of the small bowel, due to its no accessibility. This investigation constituted a multi centric clinical trial phase I-II. Forty-four patients were prospectively evaluated with the objective of determining the validity of the scintigraphy with 99mTc-Sn-CBS for the diagnosis of illnesses with ulcerations of the intestinal tube. Of the evaluated patients, 20 presented Cohns Disease (in 1 of them the scintigraphy was not useful), Idiopathic Ulcerative Colitis, 3 gastric ulcer and 20 patients were seemingly healthy, conforming these last ones the group control. Five hundred mgs of CBS were labelled with 370 MBq (10 mCi) of 99mTc and administered by oral via as unique dose. The diagnostic test had duration of three days for each patient: first day for the patient's preparation with soft diet and 20% mannitol and two days for the image registrations. The images were carried out at 2, 4, 6 and 24 hours post administered the product. All procedures were performed by experienced examiners, who were blinded to the clinical data and other results. It was determined the sensibility and specificity of this technique taking like reference test, the colonoscopy, gastroduodenoscopy, intestinal transit and/or biopsy, achieving a sensibility of 91,3% and a specificity of 90%. During the whole study an adverse event appeared, which had not very probable causation according to the scale of Karch and Lasagna. It can be concluded that the diagnostic criteria established in this study can be useful for the evaluation of inflammatory bowel diseases by scintigraphy with 99mTc-Sn-CBS. (Author)

    Full text: Radiolabelled leucocyte scans are established as an accurate method of detecting inflammatory bowel disease and focal infection but involve complex in vitro cell labelling techniques. As antibiotic peptides accumulate at sites of inflammation, we studied the localisation of the antibiotic dipeptide 99m Tc-alafosfalin in a rat model of colitis and compared it to the non-specific marker 99m Tc-DTPA. Distal colitis was induced in female Sprague-Dawley rats (175g) 4 days following the rectal infusion of 1 ml trinitrobenzylsulphonic acid (80mg/ml; 30% aq. ethanol). Six colitic and 6 control rats were each injected intravenously with 5MBq 99m Tc-alafosfalin (>90% radiolabelling efficiency) and 5MBq 99m Tc-DTPA and organ and target site uptake measured by scintigraphy and quantitative counting of excised tissue (% injected dose/gram tissue) at 1 and 4 hours. Colitis was confirmed by histological examination of rectal specimens. The scans and tissue counts showed significant focal accumulation of 99m Tc-alafosfalin in inflamed rectum compared to normal bowel (p 99m Tc-alafosfalin compared to 99m Tc-DTPA (p=0.03).   5. Altered splenic uptake in the biodistribution of 99MTc-stannous fluoride colloid in rats

    Full text: Radiolabelled white cells have found extensive application in nuclear medicine practice, with use in detecting sites of infection and inflammation. In 99M Tc-stannous flu- oride (SnF2) colloid labelled leucocyte scans high liver and spleen activity is visualised, limiting the potential of this technique. This is thought to be due to free or unphagocytosed 99M Tc-SnF2 colloid in the preparation. Labelling white cells with 99M Tc-SnF2 depends upon phagocytic engulfment of this colloid. Non-phagocy-tosed colloid may be a cause of increased splenic activity. Our aims were to assess if it were possible to reduce liver/spleen uptake of 99M Tc-SnF2 colloid in a rat model, by administering unlabelled SnF2 colloid before injection of radiocolloid, in a modification of the standard British Pharmacopoeial test. The biodistribution of 99M Tc-SnF2 colloid alone gave 85.4% uptake in the liver and 7.6% in the spleen. A pre-injection of 26.5μg cold SnF2 and then 99M Tc-SnF2 colloid resulted in decreased spleen uptake by 43%. A higher (52μg) pre-injection dose gave a greater decrease in splenic uptake by 53%. When ex vivo opsonised 99M Tc-SnF2 colloid was administered, both liver and spleen uptake was not reduced. From these results, a pre-injection of unlabelled SnF2 substantially depletes the circulating opsonin concentration, thereby decreasing the opsonisation rate of unopsonised 99M Tc-SnF2 colloid. The limiting factor is likely to be the amount of opsonisation in the plasma rather than phagocytic capacity of the macrophages of liver and spleen. In conclusion these results suggest that a pre-injection of cold stannous fluoride may reduce splenic uptake in 99M Tc-SnF2 colloid labelled white cell scans.   6. 99MTc - DMSA renal scintigraphy in the diagnosis and follow-up of acute pyelonephritis in children

    The aim of the present thesis was to define and evaluate a strategy for identification of children who are at risk of developing progressive renal lesions after acute pyelonephritis. Qualitative and quantitative evaluation standards were elaborated to improve the interpretation of DMSA scintigraphy. The normal DMSA distribution pattern, the average background uptake, and scintigraphic kidney length according to age were assessed in 95 presumably healthy kidneys. Furthermore, typical DMSA distribution patterns in acute pyelonephritis were assessed on 65 kidneys in 38 children, and typical DMSA distribution patterns of 152 kidneys with VUR in 101 children with and without previous pyelonephritis. Measurement of scintigraphic kidney length, width and volume was validated in piglets and on a kidney phantom. The scintigraphic kidney length was found to be an accurate measure of renal size, whereas kidney width and volume were less reliable, at least on small kidneys. Criteria of kidney swelling in acute pyelonephritis were defined, and found to be beneficial for identifying reinfections in the absence of clinical symptoms. In 34 children with acute pyelonephritis quantitative and qualitative DMSA scintigraphic findings were correlated to clinical symptoms and laboratory data, in the acute stage and at follow up. We found that quantitative DMSA scintigraphy in the acute stage of pyelonephritis and again after one year will identify children who are at risk of developing progressive renal lesions.   7. Development and evaluation of a novel (99mtc-labeled annexin A5 for early detection of response to chemotherapy.

    Full Text Available (99mTc-HYNIC-annexin A5 can be considered as a benchmark in the field of apoptosis imaging. However, (99mTc-HYNIC-annexin A5 has characteristics of high uptake and long retention in non-target tissues such as kidney and liver. To minimize this problem, we developed a novel (99mTc-labeled annexin A5 using a bis(hydroxamamide derivative [C3(BHam2] as a bifunctional chelating agent, and evaluated its usefulness as an imaging agent for detecting apoptosis. The amino group of C3(BHam2 was converted to a maleimide group, and was coupled to thiol groups of annexin A5 pretreated with 2-iminothiolane. (99mTc labeling was performed by a ligand exchange reaction with (99mTc-glucoheptonate. Biodistribution experiments for both (99mTc-C3(BHam2-annexin A5 and (99mTc-HYNIC-annexin A5 were performed in normal mice. In addition, in tumor-bearing mice, the relationship between the therapeutic effects of chemotherapy (5-FU and the tumor accumulation of (99mTc-C3(BHam2-annexin A5 just after the first treatment of 5-FU was evaluated. (99mTc-C3(BHam2-annexin A5 was prepared with a radiochemical purity of over 95%. In biodistribution experiments, (99mTc-C3(BHam2-annexin A5 had a much lower kidney accumulation of radioactivity than (99mTc-HYNIC-annexin A5. In the organs for metabolism, such as liver and kidney, radioactivity after the injection of (99mTc-HYNIC-annexin A5 was residual for a long time. On the other hand, radioactivity after the injection of (99mTc-C3(BHam2-annexin A5 gradually decreased. In therapeutic experiments, tumor growth in the mice treated with 5-FU was significantly inhibited. Accumulation of (99mTc-C3(BHam2-annexin A5 in tumors significantly increased after 5-FU treatment. The accumulation of radioactivity in tumor correlated positively with the counts of TUNEL-positive cells. These findings suggest that (99mTc-C3(BHam2-annexin A5 may contribute to the efficient detection of apoptotic tumor response after chemotherapy.

  8. Dual-Phase 99MTc-MIBI Parathyroid Imaging Reveals Synchronous Parathyroid Adenoma and Papillary Thyroid Carcinoma: A Case Report

    Full Text Available The possibility of a coincidental appearance of hyperparathyroidism and thyroid cancer is not often considered because of its low incidence. Here, we present a case of a 49-year-old woman with a parathyroid adenoma coexisting with two sites of papillary thyroid carcinoma. Dual-phase 99mTc-methoxyisobutylisonitrile (MIBI parathyroid imaging before the operation correctly visualized the site of the parathyroid adenoma. In addition, two papillary thyroid carcinomas showed faint uptake of 99mTc-MIBI on delayed image. Total thyroidectomy and parathyroidectomy of a solitary parathyroid adenoma were performed. The patient subsequently underwent radioiodine-131 ablation and was treated with T4 suppression. This case illustrates the need for clinical awareness of concomitant hyperparathyroidism and thyroid cancer. Dual-phase 99mTc-MIBI parathyroid imaging may be useful for detecting indolent thyroid cancer before it becomes a distinct disease.

  9. 99MTC-HL91 spect image versus 18F-FDG PET for detection of head and neck carcinoma

    Objective: Tumor hypoxia is a major complication of oncologic cell switch for chemotherapy(C/T) and / or radiotherapy(R/T). Such lesions detected by selective modem conventional examination remains difficult. 99mTc-HL91 is a potential agent for imaging hypoxic tissue in vivo. This study aimed to assess efficacy of 99mTc- HL91 in imaging of head and neck cancer and compared the result with 18F-fluorodeoxyglucose(FDG)PET. Methods: Sixteen pts with head and neck cancers (7 hypopharyngeal cancers, 4 laryngeal cancers, 5 tongue base cancers) were enrolled in this study. Primary tumors and suspicious local,regional metastases were diagnosed by clinical examination, CT/MRI , and biopsy. After intravenous injection of 740 MBq of 99mTc-HL91, whole body planar scan and regional SPECT at 2hr postinjection were performed. Tumor lesion -to- normal background(T/N) ratios with 3x3 pixels of background ROI were also measured. The reference range of T/N ratio greater 3.0 defined +, close to 2.4 defined +/- and less than 1.5 defined as -. The FDG images with dedicated PET system was performed at 4hr after completion of 99mTc-HL91 study. The visualized tumors uptake with ratio of standardized uptake value (SUV) greater than 2.5 defined as +.. less than 2.5 defined -. Results: In 7 hypopharyngeal cancers, there are 2FDG+/HL91+, 3FDG+/HL91-, 2FDG+/HL91+/-, In 4 laryngeal cancers, there are 3FDG+/HL91-, 1FDG+/HL91 +/-. In 5 tongue base cancers, there are 5FDG+/HL91-.(Table 1). The T/N ratios of all head and neck cancers in primary tumor and regional lymph nodes were ranged from l.3/1.5 and 1.1/1.2 respectively. The frequency of FDG + in hypopharyngeal cancers is 1.0,in laryngeal cancers is 1.0 and in tongue base cancers is 1.0. The frequency of HL91+/+/- in hypopharyngeal cancers is 0..58,in laryngeal cancers is 0.25.and in tongue base cancers is 0. Specificity and negative predictive value (86% and 93%) were high only when septal and other reversible defects were considered. Conclusion: We conclude that 99mTc MIBI AMPI should be the imaging modality of choice in the evaluation of IHD in symptomatic hypertensive pts with LBBB. There is a high negative predictive value for IHD in our series.Our study also highlights those patients showing Septal reversible perfusion defect with or without associated reversible defects in AMPI definitely need to undergo Coronary angiogram while pts with isolated fixed septal defect need not be subjected for the same.     Full Text Available Currently, there is a urgent need to search for new diagnostic methods that allow us to reveal malignant tumors with the overexpression of Her-2/neu with high accuracy. In recent years radioisotope methods have been actively developing to identify specific tumor targets, with antibodies being the “targeting” module.The purpose of the study. Creation of a chemically stable radiochemical compound for the imaging of cells with the overexpression of Her-2/neu.Materials and methods. The study was conducted using two human adenocarcinoma cell lines with expression (BT-474 and without expression (MCF-7 Her-2/neu. The specificity of the binding of the test complex with Her-2/neu receptor was determined by direct radiometric and planar scintigraphy. To evaluate the differences in quantitative characteristics between the groups a non-parametric Mann – Whitney test was used.Results. The yield of the labeled complex was more than 91% and the radiochemical frequency was more than 94%. When performing a visual scintigraphic evaluation, a much higher accumulation rate of the studied radiopharmaceutical preparation (RFP was observed in the culture of cells with overexpression of the surface Her-2/neu receptor. Direct radiometric results also demonstrated a higher accumulation of RFPs in the human BT-474 mammary adenocarcinoma cell line with Her-2/neu overexpression in comparison with the control group.Conclusion. Preclinical studies demonstrated high stability of the test compound, as well as its accumulation in the group of cells with Her-2/neu overexpression

  12. The prognostic value of 99MTc-MIBI stress-rest myocardial perfusion imaging in patients suspected with coronary artery disease

    Objective: To evaluate the prognostic value of 99mTc-MIBI stress-rest myocardial perfusion imaging (MPI) for patients suspected with coronary artery disease (CAD). Methods: 206 consecutive patients suspected with CAD (165 males, 41 females; age range 16-90 years, mean age 55.94±12.46 years) underwent 99mTc-MIBI stress-rest myocardial perfusion imaging (two days protocol), 57 pts with hypertension, 68 pts with hyperlipemia, 39 pts with diabetes and 62 pts with the family history of CAD. According to the imaging results, the 206 patients were divided into 3 groups. G1, normal, 135 pts; G2, reversible defects, 20 pts; G3, fixed defects, 51 pts. The mean time of follow-up was 33.25±14.95 (1-56) months. Results: The rate of cardiac events (CE) occurrence of the three groups was 1.5% (2/135), 65% ( 13/20) and 7.8% (4/51) respectively and there was significant difference between them. Kaplan-Meier survival curves analyzing the no-CE rates in the three groups, ischemic patients demonstrated a more significantly lower than normal and infarction ones in the no-CE rates(Log-rank statistic, chi-square 124.89, P <0.0001). The better predictor of future cardiac events is the reversible defect. Over all, 23.9% of the patients with abnormal stress MPI occurred cardiac events, but only 7.5% of the patients with abnormal rest ones did(chi-square 5.80, P =0.016). The prognoses of patients with normal stress MPI result were better than those with normal rest ones (1.5% versus 9.7%, chi-square 8.94, P =0.0028). Compared with rest MPI, the stress one is a better predictable method to analyze the patient' s condition and evaluate the prognosis. Conclusions: 99mTc-MIBI stress-rest myocardial perfusion imaging is a better Methods to evaluate the prognosis of patients suspected with CAD, and the rate of CE occurrence was 65% in reversible defects patients in this study.   13. Preparation of N-(3-BROMO-2,4,6-trimethylacetanilide) iminodiacetic acid and its 99MTc-complex for hepatobiliary imaging

    N-(3-bromo-2,4,6-trimethylacetanilide) iminodiacetic acid (BrTIDA) was synthesized using nitrilotriacetic anhydride prepared in situ, and lyophilized vials were prepared which contained 20 mg of BrTIDA and 0.4 mg of SnCl 2 . To evaluate the clinical efficacy of the in-house prepared lyophilized kit, a technetium-99m complex of BrTIDA was prepared; its in vivo pharmacokinetic behavior was evaluated via animal studies to assess the hepatocytic function and the functional status of the cystic duct and the gallbladder. Serial static image scans of rabbits and the biodistribution in mice injected with 99m Tc-BrTIDA revealed that none of the tissues except for the hepatobiliary system showed radioactivity concentrations, and a rapid clearance from the organs was observed. In conclusion, a lyophilized kit and its prepared 99m Tc-BrTIDA can be applied as a hepatobiliary imaging agent for the evaluation of the functional status of the hepatocytes and the patency of the biliary duct

  14. Incremental value of combined 99MTc tetrofosmin parathyroid scintigraphy, rapid intra operative PTH assays and minimally invasive radio-guided surgery (MIRS) in optimizing parathyroidectomies

    Objective: Aim of our study was to evaluate incremental diagnostic value of 99mTc Tetrofosmin parathyroid SPECT (TPSPECT), rapid intra operative quick parathyroid hormone measurement (QPTH) combined with radio guidance provided by scintillation probe (Minimally Invasive Radio guided Surgery MIRS) in parathyroid surgeries. While TPSPECT is an established investigation in preoperative workup of hyperparathyroidism patients , MIRS is a relatively new concept in Indian subcontinent. Methods: 29 pts (M: F = 18: 11), age range 16- 65 yrs (mean 41+ 9 yrs) having clinical and biochemical hyperparathyroidism underwent TPSPECT between Jan 02 -04, using 20 mci IV 99mTc Tetrofosmin. Pts with familial hyperparathyroidism , previous nodular goiters and previous neck irradiation were excluded from study. Scintigraphy comprised of immediate, delayed planar and SPECT imaging of neck and chest. Imagewise abnormal, persistent tracer uptake was considered positive for adenoma and diffuse uptake for hyperplasia. 26 pts underwent exploration (22 adenomas and 4 hyperplasias).18 pts had benefit of intraoperative QPTH. A select group (after Dec 2003) i.e. 9 pts got the assistance of radioguided probing. Peroperatively NM physician used cordless handheld gamma probe (Gamma Finder, World of Medicine, Germany) in neck and mediastinum to detect parathyroid adenoma / hyperplasias. A five fold increase in radioactive counts perceived by probe when compared to background was considered positive for parathyroid adenoma / hyperplasia. If PTH levels fell from baseline by at least 50%, the surgery was concluded as complete. Results: In all 22 pts suspected to have primary hyperparathyroidism TPSPECT identified adenomas (100 % sensitivity). While planar imaging had a sensitivity of 90.1% (20/22 pts), SPECT identified the adenoma in all pts. Interestingly only 3/22 pts had ectopic glands while 5 had more than one adenomas.     Introduction: Radionuclide Methods have been extensively used in kidney transplantation. This non-invasive technique provides quantitative analysis of total and individual renal function as well as assessment of urinary outflow tract. Aim of this study was to evaluate renal function of potential kidney-donors and follow up post-nephrectomy kidney-donors by 99mTc-DTPA renal scintigraphy using Gates protocol with extended acquisition. MATERIALS AND Methods: This study included 72 living kidney donors. In addition to routine laboratory and radiological evaluations, a 99mTc-DTPA dynamic renal scintigraphy was acquired 15 second per frame for 30 minutes. Glomerular filtration rate (GFR) was calculated based on Gates protocol. Urinary outflow tract was assayed by renogram generated and consecutive dynamic renal images of up to 30 minute. Renal scintigraphy was also performed in follow-up of 37 post-nephrectomy kidney donors. Results 6 of 72 (8.3%) living related donors were excluded from nephrectomy due to reduced GFR of either total or individual kidney. One of 6 excluded donors had one non-functional kidney. No donors have shown urinary tract obstruction. In 66 kidney donors who were indicated nephrectomy, pre-nephrectomy mean total GFR, calculated by Gates method, was 108 mi/minute that was significantly higher than GFR result of 94 mi/minute by Cockcroft and Gault prediction equation (p 0.05). These results are not significant, indicative of functional compensation occuring after unilateral nephrectomy. Conclusion: Tc-99m DTPA dynamic renal scintigraphy using Gates protocol with extended acquisition time is a valuable simple radionuclide technique which is able to assess total and individual renal function and urinary outflow tract. This technique should be used as an integral part of the pre-operative evaluation contributing to the choice of nephrectomy side in potential kidney donors and as follow-up of post-operative remaining kidney.     The aim of this SPECT study was to determine whether there is a correlation between rCBF ( 99m Tc-HMPAO) and D2 receptor binding ( 123 I-IBZM) in disorders of the extrapyramidal system and in which situation the 99 MTc-HMPAO scan could predict the outcome of the 123 I-IBZM study. 13 patients with Parkinson's syndrome and 13 patients with hyperkinetic extrapyramidal disorders were studied. In all patients the two SPECT studies were performed within 2-7 days. ROIs were placed over the basal ganglia (BG), the frontal cortex (FC) and the cerebellum (CE). The ratios BG/FC and BG/CE were calculated. In both groups the scatter was lower when the frontal cortex was used as reference region. Among the patients with hyperkinetic extrapyramidal hyperkinetic extrapyramidal disorders the two patients with Huntington's chorea had lower rCBF and D2 receptor binding compared to other hyperkinetic extrapyramidal disorders.     The increase in the number of nuclear medicine centers, both official and private in the country, as well as the increase in the number of patients, due to the effectiveness of their diagnostic and therapeutic procedures, brings out new situations that must be studied from the point of view of radioprotection. This work makes a revision in the medical literature about procedures with radioisotopes during the maternal nursing period. In general, it is recommended to stop nursing for 24 hours for 99mtc test, and to resume it after the draining of the milky content. This can be done in spite of the sensitivity of the target organ of the baby, because the dosage will be below permissible limits accepted by international agencies with respect to diagnostic test and I-131 treatment, and if continuing nursing is desired, it is recommended to use other diagnostic or therapeutic procedures before discontinuing the most important nutritional resource at this age

    785 combined diagnostic procedures are reported which were carried out on 353 patients with microscopically proven carcinoma of the prostate in order to detect metastases. X-ray films of the thorax, spine and pelvis were taken. Also bone-scintigraphy was done with 87MSr or 99MTc-polyphosphate. Additionally the alcaline, acid and prostate phosphatases were determined. A diagnostic coincidence between radiological results and bone-scan was found in 95.1% of cases. The bone-scan was false negative in 3.7%. Only in 1.2% metastases were detected earlier by the bone-scan than by x-ray examinations. Referring to the number of studies identical results were found. In roentgenologically detected metastases an increased serum level of the alcaline phosphatase was found in 61%, of the acid phosphatase in 31% and of the prostate-phosphatase in 25%.

    Full Text Available Presentamos el caso de una mujer de 66 años con diagnóstico de hiperparatiroidismo primario por adenoma paratiroideo ectópico mediastinal realizado por gammagrafía paratiroidea con 99mtc-metoxi-isobutil-isonitrito (Tc99-MIBI. Fue intervenida exitosamente mediante la técnica de videotoracoscopia. Durante el procedimiento se utilizó la medición de parathormona y a los 10 minutos de extirpado el adenoma se obtuvo un descenso mayor del 50% del basal. La sonda de detección gamma no fue efectiva in situ por la superposición del radioisótopo con el miocardio pero confirmó la radioactividad dentro del quirófano una vez extirpado el adenoma. La paciente normalizó los valores de calcemia y parathormona en el postoperatorio y permaneció normocalcémica luego de 9 meses de seguimiento. En nuestro caso, la localización prequirúrgica y el monitoreo de parathormona intraoperatoria fueron de gran utilidad para el éxito quirúrgico, la sonda detectora marcada con Tc99-MIBI no fue efectiva in situ pero confirmó la radioactividad una vez extirpado el adenoma. El tratamiento quirúrgico endoscópico por videotoracoscopia fue bien tolerado, menos doloroso que la toracotomía y acortó el tiempo de internación.We report a 66 years old woman with a diagnosis of primary hyperparathyroidism. Localization to mediastinum was obtained with parathyroid scintigraphy using 99mtc-methoxy-isobutyl-isonitrite (Tc99-MIBI. The patient was successfully operated upon by means of a videothora-coscopic approach. During the procedure serum parathormone was measured before and 10 minutes after adenomectomy, showing a more than 50% reduction from the basal level.   3. Incomplete palmar fracture of the proximal extremity of the third metacarpal bone in horses: ten cases (1981-1986)

    In 4 adult horses, simple, non displaced, incomplete fracture of the proximal extremity of the third metacarpal bone (MC3) was identified radiographically only on the dorsopalmar projection. Lameness was slight to moderate. Although nerve blocks of the foot and fetlock did not alter the lameness, high palmar regional nerve block improved the gait in 1 of the 2 horses on which it was performed. Pain on palpation or swollen distal accessory (inferior check) ligament, flexor tendons, and suspensory ligament were not found in any horse. The fracture was localized to the palmar surface of the proximal extremity of the MC3 on the basis of the intense uptake of radiopharmaceutical (99MTc-labeled sodium medronate) observed in that area during the soft tissue and delayed bone phases of a nuclear scintigraphic examination (nuclear scan) performed concurrently with radiography. Of 4 horses evaluated 6 months after the initial diagnosis, 3 had medullary sclerosis without radiographic evidence of fracture; results of follow-up nuclear scintigraphy performed in one of these horses at the same time were normal. Incomplete fracture also was suspected in another 6 adult horses with clinical lameness referable to the proximal extremity of the MC3. Although a fracture line could not be seen radiographically, trabecular hypertrophy and/or medullary sclerosis of the proximal extremity of the MC3 were detected on the dorsopalmar projection. Further, during nuclear scintigraphy, an intense uptake of the radiopharmaceutical was observed on the palmar aspect of the proximal extremity of the MC3 in all 6 horses
