    Full Text Available Background: Anal squamous intraepithelial lesions (SIL and cancers are increased in immunocompromised populations. Based upon anatomic and histologic similarities, the cervix is used as the model for anal screening. During cervical colposcopy, acetic acid (AA and Lugol׳s staining (LS result in characteristic changes that help distinguish low-grade (LSIL from high-grade (HSIL. Lesion characteristics were evaluated for their ability to distinguish anal (aLSIL from anal (aHSIL during high-resolution anoscopy after application of AA and LS. Methods: AA-stained lesions were described using standard cervical colposcopic criteria. LS was then applied and lesions were characterized as Lugol׳s-negative (L−, Lugol׳s-partial (L+/−, or Lugol׳s positive (L+ and then biopsied. Biopsies were characterized as benign, squamous atypia, LSIL or HSIL. Results: 835 anal lesions were analyzed. Sensitivity and positive predictive value (PPV for aHSIL were highest for characteristics associated with cervical (cHSIL. L− was independently associated with aHSIL (OR=4.7, 95% CI=3.4–6.7. In multiple logistic regression analysis, significant predictors of aHSIL were flat contour (OR=2.24, 95% CI=1.3–3.8, mosaic pattern (OR=2.0, 95% CI=1.4–2.9, vascular punctation (OR=1.5, 95% CI=1.1–2.1 and L− (OR=2.3, 95% CI=1.5–3.4. L− staining improved the PPV of aHSIL almost twofold in lesions that otherwise had a colposcopic impression of LSIL. Conclusions: Evaluating acetowhite lesions for contour, surface, vascularity, and LS may maximize the likelihood of identifying aHSIL.   2. The effect of preoperative Lugol's iodine on intraoperative bleeding in patients with hyperthyroidism.

    To investigate the effect of preoperative Lugol's iodine on intraoperative bleeding in patients with hyperthyroidism. This controlled, randomized, prospective cohort was carried out on 40 patients who admitted for surgery due to hyperthyroidism. Cases were randomly assigned to receive either preoperative treatment with Lugol solution (Group 1) or no preoperative treatment with Lugol solution (Group 2). Group 3 (n = 10) consisted of healthy adults with no known history and signs of hyperthyroidism. Blood flow through the thyroid arteries of patients was measured by color flow Doppler ultrasonography. Free T3, free T4, TSH, thyroid volume and the resistance index of the four main thyroid arteries were measured in all patients. There was not a significant difference between gender, preoperative serum thyroid hormone levels, or thyroid gland volumes between groups 1 and 2. The mean blood flow of the patients in Group 1 was significantly lower than values in Group 2. When age, gender, thyroid hormone, TSH, thyroid volume, blood flow, and Lugol solution treatment were included as independent variables, Lugol solution treatment (OR, 7.40; 95% CI, 1.02-58.46; p = 0.001) was found to be the only significant independent determinant of intraoperative blood loss. Lugol solution treatment resulted in a 7.40-fold decrease in the rate of intraoperative blood loss. Preoperative Lugol solution treatment was found to be a significant independent determinant of intraoperative blood loss. Moreover, preoperative Lugol solution treatment decreased the rate of blood flow, and intraoperative blood loss during thyroidectomy.

    Conclusion: Preoperative Lugol solution treatment was found to be a significant independent determinant of intraoperative blood loss. Moreover, preoperative Lugol solution treatment decreased the rate of blood flow, and intraoperative blood loss during thyroidectomy.

  4. Rescue pre-operative treatment with Lugol's solution in uncontrolled Graves' disease.

    Graves' disease is a common cause of hyperthyroidism. Three therapies have been used for decades: pharmacologic therapy, surgery and radioiodine. In case of adverse events, especially agranulocytosis or hepatotoxicity, pre-treatment with Lugol's solution containing iodine/potassium iodide to induce euthyroidism before surgery could be advocated, but this has rarely been reported. All patients hospitalised due to uncontrolled hyperthyroidism at the Karolinska University Hospital 2005-2015 and treated with Lugol's solution were included. All electronic files were carefully reviewed manually, with focus on the cause of treatment and admission, demographic data, and effects of iodine on thyroid hormone levels and pulse frequency. Twenty-seven patients were included. Lugol's solution had been chosen due to agranulocytosis in 9 (33%), hepatotoxicity in 2 (7%), other side effects in 11 (41%) and poor adherence to medication in 5 (19%). Levels of free T4, free T3 and heart rate decreased significantly after 5-9 days of iodine therapy (free T4 53-20 pmol/L, P  = 0.0002; free T3 20-6.5 pmol/L, P  = 0.04; heart rate 87-76 beats/min P  = 0.0007), whereas TSH remained unchanged. Side effects were noted in 4 (15%) (rash n  = 2, rash and vomiting n  = 1, swelling of fingers n  = 1). Thyroidectomy was performed in 26 patients (96%) and one was treated with radioiodine; all treatments were without serious complications. Treatment of uncontrolled hyperthyroidism with Lugol's solution before definitive treatment is safe and it decreases thyroid hormone levels and heart rate. Side effects were limited. Lugol's solution could be recommended pre-operatively in Graves' disease with failed medical treatment, especially if side effects to anti-thyroid drugs have occurred. © 2017 The authors.

    To reduce intraoperative and postoperative complications, using Lugol solution to preoperatively prepare patients with Graves' disease has (1) rapidly reduced the severity of thyrotoxicosis and (2) reduced the vascularity of the thyroid gland. The vascularity reduction normally accompanies reducing the severity of thyrotoxicosis. However, the effects and mechanism of Lugol solution for reducing blood flow have not been well investigated in the patients with euthyroid (normally functioning thyroid) Graves' disease. Twenty-five patients with euthyroid Graves' disease being preoperatively treated with Lugol solution for 10 days were measured, at baseline and on the operative day, for (1) superior thyroid artery blood flow; (2) systemic angiogenic factor (VEGF); and (3) systemic inflammatory factor [interleukin (IL)-16]. All three parameters were significantly (p Graves' disease. We recommend routine preoperative Lugol solution treatment for all patients with Graves' disease.

    Full Text Available Background and aim: patients with head and neck squamous cell malignancies have a higher risk of oesophageal squamous cell carcinomas. Lugol chromoendoscopy in oesophagus is a simple technique with a high diagnostic yield in premalignant lesions. The objective was to analyze its diagnostic accuracy in dysplasia and carcinoma of the oesophagus in high-risk patients. Methods: prospective study from April/2008 to January/2012 using lugol chromoendoscopy with biopsies of suspicious lugol voiding areas ≥ 5 mm. Patients with head and neck malignancies were included, except the ones with iodine allergy, oesophageal varices and contra-indications to standard endoscopy. The reference method was histopathology. Results: 89 patients were enrolled (mean age 62.8 ± 13.3 years, 87 % men. Primary tumour was located in oropharynx in 37 (41.6 %, in oral cavity in 29 (32.6 % and in the larynx in 23 (25.8 % cases. 40.4 % patients had previous treatments and 87 % reported alcohol or tobacco addition. All exams performed without anaesthesia or complications. Nine suspicious lugol voiding areas were observed and biopsied. Histopathological analysis revealed high-grade dysplasia in 2 (2.2 % and inflammation or normal findings in the others. The sensitivity and specificity for detecting high-grade dysplasia were 100 % and 92 % (95 % CI: 87-97, respectively. Diagnostic accuracy of the test was 92 % (95 % CI: 86-98. Conclusion: lugol staining of the oesophagus during endoscopy seems to be a feasible, safe and justified procedure in high-risk population as it enhances the detection of premalignant lesions.

  8. Pre-Operative Lugol's Iodine Treatment in the Management of Patients Undergoing Thyroidectomy for Graves' Disease: A Review of the Literature.

    To undertake a review of the relevant English literature published on the pre-operative use of Lugol's iodine in the management of patients undergoing thyroidectomy for Graves' disease. We reviewed all relevant papers found through Ovid Medline, PubMed, EMBASE and the American Thyroid Association website. Searches were limited to the English language only. The critical appraisal tool CASP was used to help analyse the papers. Following this, the evidence was ranked using the Harbour and Miller classification of hierarchy. Four papers were deemed appropriate for analysis. The evidence contained within the review is considered weak. The literature available in the public domain regarding the use of iodinated solutions in the pre-operative period for those patients about to undergo thyroidectomy for Graves' disease is scant. Having undertaken an extensive literature review, we are of the opinion that the evidence on which the American Thyroid Association's guidance on the use of preoperative Lugol's iodine is based is tenuous. There appears to be little in the way of sound clinical evidence that post-operative outcomes are any different following a course of Lugol's iodine. Given the lack of robust clinical evidence regarding the clinical need for iodine solution in the pre-operative period, it appears clear that a larger, prospective, randomised controlled trial of all relevant outcomes - clinical and scientific - is required to answer whether or not patient preparation with Lugol's iodine is in fact necessary prior to operative intervention for Graves' disease.

  9. High-definition endoscopy with iScan and Lugol's solution for the detection of inflammation in patients with nonerosive reflux disease: histologic evaluation in comparison with a control group.

    Nonerosive reflux disease (NERD) is commonly diagnosed in patients with symptoms of reflux. The aim of the present study was to determine whether high-definition endoscopy (HD) plus equipped with the iScan function or chromoendoscopy with Lugol's solution might permit the differentiation of NERD patients from those without reflux symptoms, proven by targeted biopsies of endoscopic lesions. A total of 100 patients without regular intake of proton pump inhibitors and with a normal conventional upper endoscopy were prospectively divided into NERD patients and controls. A second upper endoscopy was performed using HD+ with additional iScan function and then Lugol's solution was applied. Biopsy specimens were taken from the gastroesophageal junction in all patients. A total of 65 patients with reflux symptoms and 27 controls were included. HD(+) endoscopy with iScan revealed subtle mucosal breaks in 52 patients; the subsequent biopsies confirmed esophagitis in all cases. After Lugol's solution, 58 patients showed mucosal breaks. Sensitivity for the iScan procedure was 82.5%, whereas that for Lugol's solution was 92.06%. Excellent positive predictive values of 100% and 98.3%, respectively, were noted. The present study suggests that the majority of patients with NERD and typical symptoms of reflux disease can be identified by iScan or Lugol's chromoendoscopy as minimal erosive reflux disease (ERD) patients. © 2014 International Society for Diseases of the Esophagus.

  10. Antiproliferative/cytotoxic effects of molecular iodine, povidone-iodine and Lugol's solution in different human carcinoma cell lines.

    Clinical trials have revealed that molecular iodine (I 2 ) has beneficial effects in fibrocystic breast disease and in cyclic mastalgia. Likewise, povidone-iodine (PVP-I), which is widely used in clinical practice as an antiseptic agent following tumour surgery, has been demonstrated to have cytotoxic effects on colon cancer and ascites tumour cells. Our previous study indicated that the growth of breast cancer and seven other human malignant cell lines was variably diminished by I 2 and iodolactones. With the intention of developing an iodine-based anticancer therapy, the present investigations extended these studies by comparing the cytotoxic capacities of I 2 , potassium iodide (KJ), PVP-I and Lugol's solution on various human carcinoma cell lines. Upon staining the cell nuclei with Hoechst 33342, the cell densities were determined microscopically. While KJ alone did not affect cell proliferation, it enhanced the antiproliferative activity of I 2 . In addition, PVP-I significantly inhibited the proliferation of human MCF-7 breast carcinoma, IPC melanoma, and A549 and H1299 lung carcinoma cells in a concentration corresponding to 20 µM I 2 . Likewise, Lugol's solution in concentrations corresponding to 20-80 µM I 2 were observed to reduce the growth of MCF-7 cells. Experiments with fresh human blood samples revealed that the antiproliferative activity of PVP-I and I 2 is preserved in blood plasma to a high degree. These findings suggest that PVP-I, Lugol's solution, and a combination of iodide and I 2 may be potent agents for use in the development of antitumour strategies.

  11. Evaluation of double formalin--Lugol's fixation in assessing number and biomass of ciliates: an example of estimations at mesoscale in NE Atlantic.

    Ciliated protozoa are potential grazers of primary and bacterial production and act as intermediaries between picoplankton and copepods and other large suspension feeders. Accurate determination of ciliate abundance and feeding mode is crucial in oceanic carbon budget estimations. However, the impact of different fixatives on the abundance and cell volume of ciliates has been investigated in only a few studies using either laboratory cultures or natural populations. Lugol's solution and formalin are the most commonly used fixatives for the preservation of ciliates samples. In the present study, the aim was to compare 0.4% Lugol's solution and 2% borated-formalin fixation and evaluate the need of counting duplicate samples each using a different fixative. For this, a large number of samples (n = 110) from the NE Atlantic was analyzed in the frame of POMME program (Multidisciplinary Mesoscale Ocean Program). We established a statistically significant relationship (p Tintinnus spp. did not show any difference between the two treatments. Abundance and biomass of mixotrophic ciliates (chloroplast-bearing cells) were for various reasons underestimated in both treatments. Our results show that unique fixation by formalin may severely underestimate ciliates abundance and biomass although their population may not alter. For this reason, Lugol's solution is best for the estimation of their abundance and biomass. However, for counts of mixotrophs and the evaluation of the ecological role of ciliates in carbon flux, double fixation is essential. Compromises regarding the fixatives have lead to severe underestimations of mixotrophs in studies conducted by now.

    The world health organization recommends visual inspection with acetic acid (VIA) and/or Lugol's Iodine (VILI) for cervical cancer screening in low-resource settings. Human interpretation of diagnostic indicators for visual inspection is qualitative, subjective, and has high inter-observer discordance, which could lead both to adverse outcomes for the patient and unnecessary follow-ups. In this work, we a simple method for automatic feature extraction and classification for Lugol's Iodine cervigrams acquired with a low-cost, miniature, digital colposcope. Algorithms to preprocess expert physician-labelled cervigrams and to extract simple but powerful color-based features are introduced. The features are used to train a support vector machine model to classify cervigrams based on expert physician labels. The selected framework achieved a sensitivity, specificity, and accuracy of 89.2%, 66.7% and 80.6% with majority diagnosis of the expert physicians in discriminating cervical intraepithelial neoplasia (CIN +) relative to normal tissues. The proposed classifier also achieved an area under the curve of 84 when trained with majority diagnosis of the expert physicians. The results suggest that utilizing simple color-based features may enable unbiased automation of VILI cervigrams, opening the door to a full system of low-cost data acquisition complemented with automatic interpretation.

    This study aims to evaluate and to compare the performance of cervical digital photography (CDP) to the visual inspection with acetic acid (VIA) and visual inspection with Lugol's iodine (VILI) methods for screening the uterine cervix cancer and its precursor lesions in developing countries. A cross-sectional study was performed in Brazil. 176 women were evaluated by VIA, VILI, CDP with acetic acid and CDP with Lugol's iodine. Kappa statistic was used to estimate the interobserver and intermethod agreement. Sensitivity, specificity and diagnostic accuracy of the four methods (VIA, VILI, CDP with acetic acid, CDP with Lugol's iodine) was calculated. Interobserver agreement for CDP with acetic acid was K = 0.441 and for CDP with Lugol's iodine was K = 0.533; intermethod agreement of VIA and CDP with acetic acid, K = 0.559; and of VILI and CDP with Lugol's iodine, K = 0.507. Sensitivity and specificity of CDP with acetic acid were 84.00 and 95.83 %, and of CDP with Lugol's iodine were 88.00 and 97.26 %, respectively. The diagnostic accuracy of CDP with acetic acid and CDP with Lugol's iodine was 92.78 and 94.90 %, respectively. This was the first study to assess the CDP with Lugol's iodine performance, which had similar performance to the CDP with acetic acid. CDP is considered a promising method for screening the uterine cervix cancer and its precursor lesions in developing countries.

    Endocardial mapping has suggested that Purkinje fibers may play a role in the maintenance of long-duration ventricular fibrillation (LDVF). To determine the influence of Purkinje fibers on LDVF, we chemically ablated the Purkinje system with Lugol solution and recorded endocardial and transmural activation during LDVF. Dog hearts were isolated and perfused, and the ventricular endocardium was exposed and treated with Lugol solution (n = 6) or normal Tyrode solution as a control (n = 6). The left anterior papillary muscle endocardium was mapped with a 504-electrode (21 x 24) plaque with electrodes spaced 1 mm apart. Transmural activation was recorded with a six-electrode plunge needle on each side of the plaque. Ventricular fibrillation (VF) was induced, and perfusion was halted. LDVF spontaneously terminated sooner in Lugol-ablated hearts than in control hearts (4.9 +/- 1.5 vs. 9.2 +/- 3.2 min, P = 0.01). After termination of VF, both the control and Lugol hearts were typically excitable, but only short episodes of VF could be reinduced. Endocardial activation rates were similar during the first 2 min of LDVF for Lugol-ablated and control hearts but were significantly slower in Lugol hearts by 3 min. In control hearts, the endocardium activated more rapidly than the epicardium after 4 min of LDVF with wave fronts propagating most often from the endocardium to epicardium. No difference in transmural activation rate or wave front direction was observed in Lugol hearts. Ablation of the subendocardium hastens VF spontaneous termination and alters VF activation sequences, suggesting that Purkinje fibers are important in the maintenance of LDVF.

    Cervical cancer can be prevented and mortality/morbidity reduced by early detection and referral. Developing countries are likely to benefit from more cost effective methods of screening and treatment. Visual inspection with acetic acid and Lugol's iodine (VIA/VILI) offers a see and treat solution thus an affordable and efficient way to identify pre-malignant lesions. Immediate treatment with cryotherapy can be offered if pre-malignant lesions are found on visual inspection. Cryotherapyis a simple procedure that is curative for dysplasia; it is likely to benefit cervical dysplasia cases picked early in resource poor settings, however there are several factors that hinder patients' access to this noble technique. Determine hindrances to cryotherapy for patients following positive results of VIA/VILI after referral. Cross sectional Study Khwisero, Western Kenya. Women attending a medical camp, willing to get screened for cervical cancer. One hundred and nine patients were screened; seventy three (66.97%) were negative for VIA/VILI, twenty one (19.26%) were positive and referred for cryotherapy. Reasons for lack of follow up were financial constraints, lack of medical personnel at referral centres and poor access to the referral facilities.19.26% of women identified with positive lesions required intervention. No patient received cryotherapy following referral. There is urgent need for availability of cryotherapy machines and training of personnel who can perform cryotherapy at the primary care level. Regional studies on knowledge attitudes and practices about VIA/VILI and cryotherapy are required to provide reasons for the poor uptake of this procedure.

    Female mice of various gestation periods were injected intraperitoneally with 0.25-0.5O ml of rose bengal 131 I with the activity between 225-250 μCi. A group was administered with Lugol's solution one day before treatment. Accumulation of radio-rose bengal in the fetuses started at the eleventh day and great increase occured at the seventeenth day of gestation till birth. Acculmulations in both fetal and female mice's thyroids could be prevented by administering Lugol's solution before treatment, and the second target of the labelled compound after the thyroid gland was liver. In this study, amoeba prevalence in 160 cases of IBD, 130 of ulcerative colitis and 30 of Crohn's disease were investigated in fresh faeces by means of wet mount+Lugol's iodine staining, modified formol ethyl acetate and trichrome staining methods and to compare the diagnostic accuracy of wet mount+Lugol's iodine staining, modified formol ethyl acetate and trichrome staining methods in the diagnosis of Entamoeba histolytica (E. histolytica)/ Entamoeba dispar (E. dispar). E. histolytica/E. dispar cysts and trophozoites were found in 14 (8.75 %) of a total of 160 cases, 13 (10.0 %) of the 130 patients with ulcerative colitis and 1 (3.3 %) of the 30 patients with Crohn's disease. As for the 105 patients in the control group who had not any gastrointestinal complaints, 2 (1.90 %) patients were found to have E. histolytica /E. dispar cysts in their faeces. Parasite prevalence in the patient group was determined to be significantly higher than that in the control group (Fischer's Exact Test, Pparasites were compared with one another, the most effective one was found to be trichrome staining method (Kruskal-Wallis Test, Pprevalence compared to the normal population. The trichrome staining method is more effective for the detection of E. histolytica /E. dispar than the wet mount+Lugol's iodine staining, modified formol ethyl acetate methods.

    Full Text Available Entre agosto de 1990 y diciembre de 1991 se examinaron 120 muestras de materia fecal de niños o adultos que consultaron por diarrea, sugestiva de ser causada por Cryptosporidium spp. En todos los casos se realizó la coloración con Lugol-Nigroslna, que proponemos, y se hizo la confirmación con la de Ziehl Neelsen modificada, pese a su limitación de teñir con el mismo patrón de coloración el Cryptosporldium y estructuras diferentes a él. En 20 (16.6% muestras (12 de niños y 8 de adultos se identificaron ooquistes de Cryptosporldlum spp y todas se confirmaron como positivas por la coloración de Ziehl Neelsen modificada. Dado que no siempre es fácil la observación de parásitos de poca prevalencia sugerimos esta coloración como ensayo de rutina porque ayuda a distinguir los ooquistes de Cryptosporidium y mejora la observación de todos los protozoarios.

    We examined 120 stool specimens from patients with diarrheal disease, suspected of being infected with Cryptosporidium. Preliminary observation was made with a Lugol-Nigrosine stain and confirmation with modified Ziehl-Neelsen. Twenty specimens (12 from children and 8 from adults (16.6% were positive for Cryptosporidium oocysts andevery one of them was confirmed with ZN stain. Since It may be difficult to detect low-prevalence parasites we suggest routine use of Lugol-Nigrosine which is useful for the detection of Cryptosporidium as well as of other protozoa.

    Objective: To evaluate the usefullness of flexible esophagoscopy and chromoendoscopy with Lugol's solution in the detection of synchronous esophageal neoplasm in patients with hypopharyngeal squamous cell carcinoma (HSCC). Methods: A retrospective review of 96 cases with HSCC that received surgical treatment from March 2016 to March 2017 was accomplished. In these patients, the site of origin were pyriform sinus ( n =75), posterior pharyngeal wall ( n =11) and postcricoid ( n =10). Esophagoscopy was prospectively performed on all patients before treatment for HSCC. All patients underwent conventional white-light endoscopic examination with Lugol chromoendoscopy and narrow band image. Suspicious areas of narrow band image or Lugol-voiding lesions were observed and biopsied. The treatment strategy of primary HSCC was modified according to the presence of synchronous esophageal squamous cell neoplasms by a multidisciplinary approach. Results: Ninety-six patients were enrolled (age ranging from 37-80 years). All patients did not have previous treatment.Histopathological analysis revealed middle to high-grade dysplasia in 5 cases, Tis cancer in 5 cases, cancer in 16 cases and inflammation or normal findings in the others. Four cases were treated with endoscopic submucosal dissection before hypopharygeal surgery, 3 cases with lower esophageal cancers were treated with gastric pull-up combined with free jejunal flap after total circumferential pharyngolaryngectomy (TCPL) and certical esophagectomy, and 14 cases were treated with TCPL, total esophagectomy and gastric pull-up. Conclusions: Esophagoscopy is a feasible and justified procedure in HSCC cases as it enhances the detection of premalignant lesion or second primary cancer. Routine esophagoscopy for detecting synchronous second primary tumor should be recommended for patients with HSCC. The treatment strategy for primary HSCC is modified according to the presence of synchronous second primary tumor.

    Thirty-nine samples of honey produced in Goiânia-GO and vicinities were analysed in relation to solid solubles, moisture, acidity in formic acid reducing sugars on glucose. The search for falsification was based on LUND’s reaction, the FIEHE test, Lugol test and the determination of the diastasics ferments. Only 30.77% of the samples analyzed were within the established by the legislation.

    Foram analisadas 39 amostras de mel de abelha produzido na região de Goiânia – GO, com o objetivo de se avaliar sua qualidade. Determinou-se: sólidos solúveis, umidade, acidez em ácido fórmico e glicídios redutores em glicose. As pesquisas de fraudes basearam-se nas provas de FIEHE, de LUND, teste do lugol e determinação de fermentos diastásicos. Somente 30,77% das amostras analisadas apresentaram-se conforme os padrões estabelecidos pela legislação vigente em todas as provas realizadas.

    Drifting agents accelerate the exchange process and thus promote to eliminate radioactive materials from human body. The earlier is the administration of the agent, the more effective is the elimination. Against the uptake of radioiodine by thyroid, anti-thyroid drug like NaI, Lugol`s iodine solution, propylthiouracil and methimazole are recommended. Ammonium chloride can be a solubilizer of radioactive strontium. Diuretics may be useful for excretion of radioisotopes of sodium, chlorine, potassium and hydrogen through diuresis. Efficacy of expectorants and inhalants is not established. Parathyroid extract induces decalcification and thus is useful for elimination of 32P. Steroids are used for compensating adrenal function and for treatment of inflammation and related symptoms. Chelating agents are useful for removing cations and effective when given early after contamination. EDTA and, particularly, DTPA are useful for elimination of heavy metals. For BAL (dimercaprol), its toxicity should be taken into consideration. Penicillamine is effective for removing copper and deferoxamine, for iron. Drugs for following radioisotopes are summarized: Am, As, Ba, Br, Ca, Cf, C, Ce, Cs, Cr, Co, Cm, Eu, fission products, F, Ga, Au, H, In, I, Fe, Kr, La, PB, Mn, Hg, Np, P, Pu, Po, K, Pm, Ra, Rb, Ru, Sc, Ag, Na, Sr, S, Tc, Th, U, Y, Zn and Zr. Lung and bronchia washing are effective for treatment of patients who inhaled insoluble radioactive particles although their risk-benefit should be carefully assessed.     A study of incidence rates of thyroid carcinoma was conducted in Olsztyn province from 1 January 1994 to 31 December 2003 within its former boundaries, in spite of Poland's new administrative division. The criteria for register entry were as follows: residence in Olsztyn province, newly-diagnosed case of thyroid malignancy in the given calendar year, and histopathological verification in the Department of Anatomical Pathology of the District Specialist Hospital in Olsztyn. The study of selected risk factors comprised patients included in the register of thyroid carcinoma. For that purpose a questionnaire was prepared which covered information about the Chernobyl accident: place of residence, time of carcinoma diagnosis after the accident, and iodine prophylaxis during the accident. The control group consisted of 589 healthy subjects selected based on age and place of residence. In the years 1993-2003, 462 (395 women and 67 men) cases of thyroid cancer were registered. The questionnaire study comprised 297 patients with thyroid carcinoma and 589 healthy subjects. Study subjects from both the affected and control groups stayed mainly in their place of residence during the Chernobyl accident (97.28% v. 94.24%). Thyroid carcinoma was diagnosed on average 13.58 ± 2.61 years after irradiation. There were no significant differences in iodine prophylaxis during the Chernobyl accident. Lugol's solution was given to 31% of patients and 34% of healthy respondents. 1. It cannot be stated that the Chernobyl disaster had any influence on the incidence rate of thyroid carcinoma in the province of Olsztyn. 2. Iodine prophylaxis using Lugol's solution could have an influence on lack of significant increase of the thyroid carcinoma incidence rate in the age group 1-18 years.

  1. A case of Dipylidium caninum infection in a child from the southeastern Poland.

    Dipylidium caninum is a common intestinal tapeworm of dogs, cats and foxes. However, it occasionally infects also humans. We present a case of D. caninum infection in a 2-year-old child living in the Subcarpathian province. The infection was asymptomatic in the first months. The symptoms of abdominal pains, sleep disorders, loss of appetite, hyperactivity and occasional slimy stools appeared later. Proglottids on the underwear, in water while bathing and mobile proglottids passed with the stool were also observed. Prior to appropriate diagnosis the child was treated with pyrantelum (Pyrantelum) and albendazolum (Zentel). However, proglottids were found again in the stool after a few days. We examined stool samples and perianal smears collected from the child and his family. The stool samples were tested by coproscopic methods. Direct methods (direct preparation in 0.9% sodium chloride and in Lugol's solution, Kato thick smear) and concentration methods (decantation with distilled water and Faust's zinc sulphate centrifugal flotation) were used. In the stool samples taken from the child, we observed D. caninum proglottids demonstrating lateral genital pores and many packets of eggs containing from one to a few, mostly 3 to 4 eggs. In the direct preparations in 0.9% sodium chloride and in Lugol's solution single packets with D. caninum eggs were detected. In decantation preparations many D. caninum egg packets were observed. It has to be reported that the child's mother was infected with Giardia intestinalis. Dipylidiasis in humans is a rarely encountered infection in Poland and the diagnosis may be difficult. For these reasons we reported clinical case presentation, diagnostics, treatment and epidemiology of D. caninum infection. We have shown that concentration methods such as decantation might be very helpful in the diagnosis of dipylidiasis.

    Full Text Available The objective of the present study was to determine the prevalence of the intestinal parasites most commonly found in immunocompromised patients. A group of 111 individuals with acute lymphoid leukaemia (ALL, chronic myeloid leukaemia (CML, human immunodeficiency virus (HIV and other immunocompromised conditions (principally haematological disorders was selected. A battery of tests was performed on each individual to identify the presence of parasites (three stool specimens with saline solution and Lugol both directly and by concentration, culture and special staining. No significant differences were found among the frequencies of the different parasites with the several types of immunocompromised conditions. The overall frequencies of potentially pathogenic and opportunistic parasites were 32.4% (36/111 and 9% (10/111 respectively, the most frequently encountered among the latter being Cryptosporidium sp., Microsporidia spp. and Strongyloides stercoralis.O objetivo do presente estudo é avaliar a frequência de parasitas intestinais em pacientes com alterações imunológicas. Foram analisadas 111 pessoas com leucemia linfóide aguda (LLA, leucemia mieloide crônica, virus da imunodeficiência humana (VIH e outras doenças, principalmente alterações sanguíneas. Para cada indivíduo foram realizados exames seriados de fezes tanto com solução salina quanto com lugol #3, direto e por concentração, culturas de fezes e colorações específicas. Não houve diferenças significativas na frequência de parasitoses nos distintos grupos avaliados. A frequência de parasitas potencialmente patogênicos foi de 32.4% (36/111 enquanto a de parasitas oportunistas foi de 9.0% (10/111. As espécies mais observadas foram C. parvum, microsporídios spp. e S. stercoralis.

    Nowadays a growing number of exotic reptiles are kept as pets. The aim of this study was to determine the species of parasites found in reptile patients of veterinary practices in Poland. Fecal samples obtained from 76 lizards, 15 turtles and 10 snakes were examined by flotation method and direct smear stained with Lugol's iodine. In 63 samples (62.4%) the presence of parasite eggs and oocysts was revealed. Oocysts of Isospora spp. (from 33% to 100% of the samples, depending on the reptilian species) and Oxyurids eggs (10% to 75%) were predominant. In addition, isolated Eimeria spp. oocysts and Giardia intestinalis cysts were found, as well as Strongylus spp. and Hymenolepis spp. eggs. Pet reptiles are often infected with parasites, some of which are potentially dangerous to humans. A routine parasitological examination should be done in such animals.

    Phytoplankton samples are routinely taken at every station on Brookhaven oceanographic cruises. On productivity stations samples are taken from every depth, while on hydrographic stations samples are taken from the surface and some fixed depth (e.g. 20 m). The 125 ml samples are drawn as soon as possible after the cast from Niskin bottles and preserved with 2-3 ml of Lugol's solution. The samples are left in dark boxes in cool areas until counted. For counting, 10 ml subsamples are settled in Uttermohl chambers, usually within three months after the cruise, and counted with an inverted microscope equipped with phase contrast illumination. Between one-half and the entire chamber are counted, depending on the cell density. This data report is a listing of the phyto- plankton counts from selected stations and cruises between 1976 and 1978, inclusive.

    Full Text Available O consumo de hortaliças cruas constitui importante meio de transmissão de enteroparasitoses. O objetivo deste trabalho foi avaliar a contaminação parasitária em alfaces (Lactuca sativa comercializadas na cidade de Guarapuava (PR. No período de julho/2005 a junho/2006, 52 supermercados foram visitados, com colheita de um pé de alface. A análise parasitológica foi realizada após duas lavagens, filtragem em gaze e sedimentação. O sedimento, corado pelo lugol, foi analisado microscopicamente por exame direto e após centrífugo-flutuação em sulfato de zinco. Das 52 amostras de alface avaliadas, 18 (34,6% apresentaram uma ou mais estruturas parasitárias como cistos de Giardia spp. (7,7%, de Entamoeba spp. (11,5% e de Balantidium coli (3,8%, oocistos de coccídios (7,7% e ovos de Capillaria spp. (3,8%. Esse alto número de amostras contaminadas aponta deficiências na cadeia produtiva dessas hortaliças, indicando a necessidade de adequada higienização antes de seu consumo. The aim of this study was to evaluate the parasitological contamination of lettuce (Lactuca sativa commercialized in Guarapuava City, Parana State. Fifty-two samples of lettuce were collected from supermarkets during the period of July 2005 and June 2006. Parasitological analysis was realized by the sedimentation technique. Lugol-stained sediments were microscopically analyzed by direct exam and after centrifugation-fluctuation in zinc sulfate. A total of 18 out of 52 lettuce samples analyzed (34.6% were positive for one or more parasitic structures. The parasites found in the analysis were Giardia spp. (7.7%, Entamoeba spp. (11.5%, Balantidium coli (3.8%, coccidian oocysts (7.7% and Capillaria spp. eggs (3.8%. Pemeriksaan tinja dilakukan terhadap 66 anak dengan metode langsung menggunakan eosin dan lugol. Tujuan penelitian ini adalah untuk mengetahui gambaran infeksi protozoa intestinal pada anak binaan rumah singgah Amanah.Telah dilakukan penelitian di Rumah Singgah “Amanah”, Kelurahan Rimbo Kaluang, Kecamatan Padang Barat, Kota Padang, terhadap anak biaHasil penelitian ini menunjukkan bahwa anak-anak yang terinfeksi protozoa intestinal sebesar 40,91%. Berdasarkan jenis spesies, distribusi frekuensi terbanyak yang menginfeksi anak adalah G. lamblia yaitu 37,88%, sedangkan infeksi oleh E. histolitika adalah 3,03%. Frekuensi infeksi G. Lamblia lebih tinggi pada umur < 10 tahun yaitu 27,27%, tetapi pada infeksi E. histolitika terlihat tidak ada perbedaan. Distribusi infeksi berdasarkan jenis kelamin hampir sama pada G. lamblia maupun E. histolitika.     The purpose of this study was to investigate the prevalence of house rat zoonotic intestinal parasites from Surabaya District, East Java, Indonesia that have the potential to cause opportunistic infection in humans. House rat fecal samples were collected from an area of Surabaya District with a dense rat population during May 2015. Intestinal parasites were detected microscopically using direct smear of feces stained with Lugol's iodine and modified Ziehl-Neelsen stains. The fecal samples were also cultured for Strongyloides stercoralis. Ninety-eight house rat fecal samples were examined. The potential opportunistic infection parasite densities found in those samples were Strongyloides stercoralis in 53%, Hymenolepis nana in 42%, Cryptosporidium spp in 33%, and Blastocystis spp in 6%. This is the first report of this kind in Surabaya District. Measures need to be taken to control the house rat population in the study area to reduce the risk of the public health problem.     Differential identification of Entamoeba histolytica and Entamoeba dispar is essential for both appropriate patient treatment and epidemiological purposes. To determine the prevalence of these amoeba infections in Santa Rosa de Agua (Maracaibo, Zulia State, Venezuela), a PCR assay using specific primers for each species was standardized and applied. 204 stool samples were analyzed through direct microscopic examination with SSF (0.85%) and lugol, formol-ether concentration, and PCR. Under direct microscopy, 42 individuals (20.58%) presented the E. histolytica/E. dispar complex. Meanwhile PCR showed 47 positive cases for these amoebas: 22 E. histolytica (10.78%), 16 E. dispar (7.84%), and 9 (4.41%) mixed infections. There was no significant difference in the presence of E. histolytica and/or E. dispar according to either gender or age. There were no cases of these amoebas in children under 2 years of age. Observed frequency of E. histolytica (31/204) shows the endemic nature of amoeba infection in this community.

    Immunoscintigraphy with CEA monoclonal antibodies (MoA) in patients with colorectal cancer has been applied since 1987 by the authors. MoA from the hybridoma F023C5 are used (IgG 1 -class) and their fragments labelled with 131 I and 111 In. The labelled MoA are introduced intravenously in the course of 30 min, the total activity is 2.5 - 3.5 mCi. The scanning is made 48 and 96 hours on gamma camera. An additional activity on 99m Tc-sulfocolloid and 99m Tc-DTPA is applied for outlining the liver and kidney contours. Digital substraction technique is applied for image processing with contrast and background reduction. The thyroid is blocked with Lugol solution in a course of 5-6 days. Among all of the 18 investigated patients a positive result has been observed in 16. Metastases bigger than 1 cm have a positive scan.     Full Text Available ABSTRACT This study aimed to quantify and identify the profile of the rumen protozoa population of beef steers fed with or without roughage. Nellore crossbred steers raised in extensive system on lignified tropical pastures with mineral supplementation and steers confined for 60 days only receiving pelletized concentrate and whole corn kernels were evaluated. After slaughter, rumen fluid was collected and one ml aliquots were diluted in nine ml formaldehyde solution at 10%. The counts of small, medium and large protozoa were held in Sedgewick Rafter chambers and identification of genus was possible after staining lugol and optical microscope with a 40X objective. The concentration of rumen protozoa positively correlated with pH ruminal. Cattle fed without roughage had significantly lower rumen protozoa population (P<0.05. Animals fed roughage had higher occurrence of Dasytrichia genus, Charonina, Entodinum, Diplodinium, Ostracodinium and Epidinium while those fed without bulk, the Buetschilia, Isotricha, Eodinium, Polyplastron, Elyplastron, Metadinium and Enoploplastron were the most frequent genus.

    Full Text Available O mel contém principalmente açúcares em alta concentração, provenientes do néctar das flores, e a própolis é coletada principalmente das folhas, brotos, exsudatos e resinas das mais variadas espécies botânicas. Tanto a composição química do mel quanto a da própolis podem variar conforme uma série de fatores, que podem estar direta ou indiretamente relacionados com as abelhas e o meio em que vivem. No entanto, esse produto pode sofrer adulterações e/ou problemas relacionados à qualidade ou adição de ingredientes de baixo valor comercial. Na tentativa de detectar esse problema, este trabalho teve como objetivo realizar um estudo de algumas características físico-químicas do mel com própolis, com o intuito de verificar possíveis alterações ou adulterações em amostras comerciais do Estado de São Paulo. Foram adquiridas onze amostras comerciais de mel com própolis e realizadas as análises: umidade, HMF, açúcares redutores, sacarose aparente, proteínas, reações de Fiehe, Lugol e Lund. Os resultados obtidos nas análises das amostras indicaram que todas elas continham mel verdadeiro e que não houve indícios de adulteração do produto com água, amido ou açúcar comercial.Honey mainly consists of sugars at high concentrations from the nectar of flowers and propolis is formed from plant secretions.     Full Text Available Abstract Background There is scant information on whether Human Immunodeficiency Virus (HIV seropositivity has an influence on the outcome of treatment of precancerous cervical lesions using cryotherapy. We studied the prevalence of cervical abnormalities detectable by visual inspection and cervical lesions diagnosed by colposcopy according to HIV serostatus and described the outcomes of cryotherapy treatment. Methods Trained nurses examined women not previously screened for cervical cancer using visual inspection with acetic acid (VIA and Lugol's iodine (VILI in two family planning/post natal clinics in Kampala, Uganda, from February 2007 to August 2008. Women with abnormal visual inspection findings were referred for colposcopic evaluation and HIV testing. Women with precancerous cervical lesions detected at colposcopy were treated mainly by cryotherapy, and were evaluated for treatment outcome after 3 months by a second colposcopy. Results Of the 5 105 women screened, 834 presented a positive screening test and were referred for colposcopy. Of these 625 (75% returned for the colposcopic evaluation and were tested for HIV. For the 608 (97.5% women in the age range 20-60 years, colposcopy revealed 169 women with cervical lesions: 128 had inflammation, 19 had low grade squamous intraepithelial lesion (LGSIL, 13 had high grade squamous intraepithelial lesion (HGSIL, 9 had invasive cervical cancer and 2 had inconclusive findings. Detection rates per 1 000 women screened were higher among the older women (41-60 years compared to women aged 20-40 years. They were accordingly 55% and 20% for inflammation, 10% and 2% for LGSIL, 5% and 2% for HGSIL, 6% and 1% for invasive cervical cancer. Of the 608 women, 103 (16% were HIV positive.     Full Text Available This study aimed to assess the role of endoparasitic infections as another factor of biological pressure on both migratory and local bird species. The occurrence of endoparasites may be considered a sign of undesirable hygienic alterations, causing the shortening of natural fields, compromising the resting area and the habitat nutritional quality. A total of 75 fecal samples were collected at the Campus of the Universidade Federal Rural do Rio de Janeiro (UFRRJ, Brazil, between August 2007 and May 2008, and they were analyzed in a Lugol´s solution by Faust´s technique. Amongst the 82.66% positive samples for enteric parasites, 44% were parasitized by the trichomonads and 5.33% by the Chilomastix enterozoary genera, 52% presented coccidian oocysts, 29.33% showed Entamoeba coli, 37.22% presented Iodamoeba butschlii vegetative forms, and 10.66% were positive for worm eggs. The high prevalence of enteroparasites observed indicates the need of more detailed and frequent evaluations of the environmental sanitary conditions, whose impact seems to be associated to the scarce local hygienic conditions due to the growing anthropization.

  15. A case report of thyroid storm induced by acute sepsis

    Full Text Available Thyroid storm is a rare but life-threatening condition, which can be induced by many critical diseases. We reported a 40-year-old woman with thyroid goiter manifesting with acute sepsis-induced hyperthyroidism. She mainly presented with abdominal bloating, diarrhea, lower limbs edema and exertional dyspnea. The lactate was 9.5 mmol/L and procalcitonin was 3.8 ng/mL, suggesting acute sepsis. The thyroid echo showed bilateral thyroid goiter. Relevant data included a thyroid-stimulating hormone level of 0.03 μIU/mL; free tetraiodothyronine, 5.67 ng/dL; thyroid-stimulating hormone receptor antibody, 76.9% (normal range, < 14%; and antimicrosomal antibody titer, 1:102400 (normal range, < 1:100, suggesting toxic goiter with thyroid storm. Piperacillin/tazobactam, methimazole and Lugol's iodine achieved a good outcome. The symptoms of early sepsis and those of thyroid storm could be similar. Therefore, a careful history taking, a thorough physical examination and a high degree of suspicion could make early diagnosis and appropriate treatment.

    Full Text Available Honey from stingless bees (Melipona sp. is a nutritious and medicinal product economically valued in the informal Brazilian market, driven by the growing demand for natural products; however, its physicochemical characteristics are still unknown. Thus, the aim of this study is to evaluate the physicochemical profile of the stingless bee honey produced in Cruzeiro do Sul, Acre. Honey samples were analyzed following the methodology recommended by Ministério da Agricultura, Pecuária e Abastecimento; the following parameters were established: moisture, total sugars, reducing sugars, apparent sucrose, ash, crude protein, diastase activity, Brix degrees, free acidity, lactonic acidity, total acidity, pH, hydroxymethylfurfural, electrical conductivity, color and Lugol, Lund and Fiehe reactions. Results show that these parameters are not suitable for all samples, being incompatible with established standards, indicating that the current legislation on Apis mellifera is not suitable for all characters analyzed, mainly moisture content, corroborating the need to standardize guidelines for honey from stingless bees. Moreover, there was no tampering in the honeys analyzed.

    About 1-2% of infants born to mothers with Graves' disease or Hashimoto's thyroiditis develop neonatal hyperthyroidism because of transplacental passage of IgG stimulating TSH receptors (TRAb). To evaluate the effect of maternal total thyroidectomy on neonatal clinical course. We describe two brothers born to a mother with Graves' disease, before and after total thyroidectomy. The first child showed persistent tachycardia, the presence of TRAb and a laboratory pattern of hyperthyroidism. Lugol's solution was started and then propylthiouracil was added. Digitalis, furosemide and diazepam were necessary for treatment of heart failure, hypertension and irritability. On the 70th day of life, hormone serum levels normalized and treatment was interrupted. TRAb normalized by the third month of life. The second infant was born 2 years after the mother underwent total thyroidectomy. In spite of a laboratory pattern of hyperthyroidism and positivity to TRAb, he showed only considerable weight loss, and no therapy was required. TRAb may persist after total thyroidectomy: clinical and instrumental follow-up of the newborn is recommended.

    The modern approach to cervical cancer prevention, characterized by use of cytology and multiple visits for diagnosis and treatment, has frequently proven challenging and unworkable in low-resource settings. Because of this, the Alliance for Cervical Cancer Prevention (ACCP) has made it a priority to investigate and assess alternative approaches, particularly the use of visual screening methods, such as visual inspection with acetic acid (VIA) and visual inspection with Lugol's iodine (VILI), for precancer and cancer detection and the use of cryotherapy as a precancer treatment method. As a result of ACCP experience in providing training to nurses and doctors in these techniques, it is now widely agreed that training should be competency based, combining both didactic and hands-on approaches, and should be done in a clinical setting that resembles the service-delivery conditions at the program site. This article reviews ACCP experiences and perceptions about the essentials of training in visual inspection and cryotherapy and presents some lessons learned with regard to training in these techniques in low-resource settings.

    Full Text Available De acordo com a tonalidade as amostras de mel foram divididas em grupos, armazenadas em diferentes condições e analisadas segundo os métodos adotados pelo Instituto Adolfo Lutz, Laboratório Nacional de Referência Animal e Association of Official Analytical Chemists. As amostras foram acondicionadas em recipientes de vidro e de polipropileno e armazenadas à temperatura ambiente. Um grupo de amostras foi armazenada em luminosidade ambiente enquanto o outro grupo ao abrigo da luz. As determinações de umidade, pH, acidez, cor, açúcares redutores e não-redutores, hidroximetilfurfural e índices de Lugol, Lund, Fiehe e diastase foram efetuadas por um período de um ano de armazenamento. Os resultados obtidos indicam que as amostras analisadas mantiveram suas características dentre os padrões aceitáveis e que os produtores devem ser orientados para o uso de técnicas de acondicionamento e armazenamento dos méis, para garantir a qualidade dos seus produtos.

    Full Text Available The aim of this study was to determine the presence of Tritrichomonasfoetus in two dairy herds on the Altiplano Cundiboyacense. Twenty-one low-volume uterine lavages from cows with a history of reproductive problems in two dairy herds located in the municipality of Sibaté (Cundinamarca and Ventaquemada (Boyacá were evaluated. In the first herd, 10 cows were sampled and in the second, 11 cows, based on three inclusion criteria. The uterine lavages were obtained through infusion of physiological saline solution into the uterine body. The samples were centrifuged and seeded in Tritrichomonas basal medium for 10-15 days at 37 ºC. The protozoa were evaluated on the day of sampling and 10 and 15 days after incubation by means of direct viewing under a dark-field microscope. Positive samples were stained with Wright and Lugol to identify the morphological characteristics. This study showed that T. foetus was present in 61.8% of the animals sampled. The determination that T. foetus was present in 61.8% of the samples analyzed is a significant finding given that in the herds evaluated, this agent had not previously been diagnosed.O objetivo do presente estudo, foi determinar a presença de Tritrichomonas foetus em dois rebanhos leiteiros no Altiplano Cundiboyacense.     We investigated the prevalence of geohelminth parasites in farmers, education concerned and shepherd of Swat, Khyber Pakhtunkhwa, Pakistan. A total of 1041 stool samples were examined from January 2006 to December 2008 using direct smear (Normal saline and Lugol's Iodine solution) the concentration methods and procedures. Seven hundred and sixty three (73.2%) individuals were found infected with one or more than one geohelminth parasites. Four hundred and eighteen (54.7%) were infected with single parasite and three hundred forty five (45.3%) with multiple infections. Ascaris lumbricoides 460 (53.0%), Trichuris trichura 228 (26.2%), Enterobius vermicularis 123 (14.1%) and Ancylostoma duedenale 56 (6.45%) were detected. The adults were found more parasitized than children and males were more infected than females. Shepherds were found more infected than farmers and education concerned. Although Swat is an area with poor hygiene located in temperate zone near the border of Afghanistan and China. The prevalence of reported geohelminth parasites here compared with the same studies is unexpectedly high. These types of studies should continue time to time to know the hazardous of such parasitic infections for the betterment of the human health.

  2. Histochemistry and morphoanatomy study on guava fruit during ripening

    Full Text Available Guava (Psidium guajava L. is a highly perishable fruit due to its intense metabolism during ripening. Information on the enzyme activities that degrade pectic substances, as well as the amount of pectin, is very contradictory and not clearly defined. Thus, this study aimed to monitor the changes occurred in the fruit during ripening through histochemical, physical, and scanning microscopy processes. Guavas were picked at the half-mature stage and stored for 9 days at 22 ± 1 °C and 78 ± 1% RH. The analyses conducted on the day of harvest (0 and each day of storage (1, 2, 3, 4, 5, 6, 7, and 8 days were: firmness and histochemical analyses (ferric chloride, lugol, comassie blue, vanillin hydrochloric, and ruthenium red observed under an optic microscope and a scanning electron microscope. Ruthenium red showed a high amount of pectin in the cell wall on day zero as well as its decrease in the wall during ripening and its accumulation in the central area of the cell. Scanning microscopy showed loss of the cell structure during ripening. Those observations suggest that the pectin is the main polymer responsible for firmness maintenance in the guava fruit.

  3. [Detection of Entamoeba histolytica and Entamoeba dispar by PCR in children, less than five years of age with diarrhea, in Maracaibo, Venezuela. A preliminary study].

    To determine the prevalence of Entamoeba histolytica as a producer of diarrhea, a study was conducted in children, less than five years of age, with diarrhea who attended several out patient clinics of the Servicio Aut6nomo Hospital Universitario, Maracaibo, Venezuela. A macroscopic and microscopic examination with physiological saline, lugol and Kinyoun staining were performed to the stool samples obtained. The remainder of the sample was frozen until DNA extraction, and PCR amplification was performed separately for E. histolytica and E. dispar. Microscopic examination showed no trophozoites and/or cysts of Entamoeba histolytica/dispar/moshkovskii, or intestinal coccidians in any of the 50 samples analyzed. Parasites detected were Giardia lamblia (6%), Blastocystis sp. (4%), Pentatrichomonas hominis (2%), Ascaris lumbricoides (2%) and Trichuris trichiura (2%). By PCR, six samples (12%) had DNA of E. dispar and two (4%) had DNA from E. histolytica; no child showed association of both amoebae. The two children who had E. histolytica were one-year-old. E. dispar was detected in younger children. We suggest that the prevalence of E. histolytica in children under five years is really low.

    Full Text Available El objetivo del trabajo fue obtener bioetanol de los residuos del material lignocelulósico de la corteza de yuca (Manihot esculenta Crantz del procesamiento de almidón, en San Pablo de Tarugo, sometidos a pre tratamiento. Se hidrolizó el material con dos concentraciones de ácido muriático (75% y 100%, tres tiempos de reacción (20, 30 y 40 min a una temperatura de 60ºC en una relación peso – volumen 1/10. Se efectuó la identificación cualitativa (ensayo de lugol, Benedict y Fehling y cuantitativa de los azúcares reductores. Con los resultados obtenidos se logró determinar el mejor tratamiento según la cantidad de azúcares reductores mediante el análisis de varianza. Obteniéndose producción de azúcares reductores de 41.73 g/L por hidrólisis con ácido muriático al 100% y tiempo de reacción de 40 minutos, fermentándose con inóculo de 7 g/L de levadura comercial Levapan (Saccharomyces cerevisae a los 30 días se obtuvo 17ºGL.

  5. Prevalence of zoonotic intestinal parasites in household and stray dogs in rural areas of Hamadan, Western Iran.

    Zoonotic parasitic infections are a major global public and veterinary health problem and widespread among dogs. The objective of this study was to assess the prevalence of intestinal parasites in stray and household dogs in the rural areas of Hamadan district. During 2012, 1,500 fresh fecal samples from 243 household and 1,257 stray dogs were examined by using direct wet mount, simple zinc sulfate flotation, and Lugol's solution staining. Of 1,500 dogs, 20.4% were positive for intestinal parasites. Helminthes eggs were more frequently found in fecal samples than protozoan cysts or trophozoites (15.9% vs. 4.5%, respectively). Toxocara canis was the most frequently detected parasite, with a prevalence of 6.3%, followed by Taenia/Echinococcus spp. (2.9%), Isospora spp. (2.7%), and Toxascaris leonina (2.6%). Helminthes and protozoa were significantly more prevalent in household dogs than in stray dogs (Pparasites indicated that people residing in this area are at risk of exposure to these potentially hazardous zoonotic pathogens. Mass education of the general population is highly recommended to increase awareness of the potential for horizontal transmission of these parasitic infections from dogs to humans.

  6. Water quality and communities associated with macrophytes in a shallow water-supply reservoir on an aquaculture farm.

    Plankton communities and macrofauna associated to aquatic macrophyte stands in a shallow water-supply reservoir (21°14'09″S; 48°18'38″W) on an aquaculture farm were compared to evaluate the relationship between organism densities and some abiotic features of the reservoir. Water and communities associated were sampled at two sites, one in an area with the predominance of Eichhornia azurea (Sw.) Kunth and the other with the predominance of Salvinia auriculata Aublet. Communities associated with macrophytes were sampled with floating quadrants (0.5 m2); the macrophytes were washed and plankton and macrofauna were fixated with 4% formalin and 1% lugol iodine; the specimens were then identified and counted. Plankton and macrofauna communities associated with S. auriculata and E. azurea had a similar diversity of species but different (pmacrophytes presence in the shallow reservoir is a strong predictor of favourable conditions to maintain great diversity plankton community and macrofauna associated with plants. The role of macrophytes is important for not only stabilising the clear-water state and maintaining high diversity of organisms associated, but also it seems to be a good alternative to maintaining desirable water-supply quality for aquaculture farms.

  7. Significance of endoscopic screening and endoscopic resection for esophageal cancer in patients with hypopharyngeal cancer

    The efficacy of endoscopic screening for esophageal cancer in patients with hypopharyngeal cancer remains controversial and its impact on prognosis has not been adequately discussed. We studied the use of endoscopic screening to detect esophageal cancer in hypopharyngeal cancer patients by analyzing the incidence, stage and prognosis. We included 64 patients with hypopharyngeal cancer who received radical radiotherapy at our institute. Chromoendoscopic esophageal examinations with Lugol dye solution were routinely performed at and after treatment for hypopharyngeal cancer. Twenty-eight esophageal cancers were detected in 28 (41%) patients (18 synchronous and 10 metachronous cancers). Of the 28 cancers, 23 were stage 0 or I cancer and 15 of these were treated with endoscopic resection. Local control was achieved in all of these 23 stage 0 or I cancers. The 5-year overall survival rates with esophageal cancer were 83% in stage 0, 47% in stage I and 0% in stage IIA-IVB. This study showed a strikingly high incidence of esophageal cancer in hypopharyngeal cancer patients. We suppose that the combination of early detection by chromoendoscopic examination and endoscopic resection for associated esophageal cancer in hypopharyngeal cancer patients improve prognosis and maintain quality of life. Population move-ments, inadequate infrastructure, seasonal features, tra-ditional hygienic rules, the society’s socio-economic sta-tus and education level are factors that affect the distribu-tion of intestinal parasites. In the study, it was intendedto conduct porter analysis on Malatya State Hospital em-ployees. So, we aimed at determining the rate of intestinalparasites in the laboratory workers, kitchen staff, cleanersand nurses.Materials and Methods: From Malatya State hospitalstaff, perianal area materials and stool samples with cel-lophane tape method were collected. Examples wereexamined with native-Lugol, precipitation, and acid-fasttrichrome stains.Results: In 40.8% of 76 stools that were examined wasfound to positivity. The prevalences of parasites are 17.1Entamoeba coli, 6.6% Iodamoeba butschlii, 19.7% Blastocystishominis, 1.3% Chilomastix mesnilii, 5.3% Giardiaintestinalis and 1.3% Enterobius vermicularis.Conclusion: In the study, the studied staffs are healthworkers. Therefore, since the staffs working close contactwith patients were risk group in terms of infections, it wasrecommended that health staff susceptible to parasitesshould have a medical examination regularly and receivein-service training.

    Full Text Available O presente trabalho analisou a composição físico-química e origem botânica de 31 amostras de mel indicadas como sendo de origem monofloral, comercializadas e produzidas na região Sudeste do Brasil, a fim de verificar parâmetros de qualidade. As análises físico-químicas compreenderam o teste de Fiehe, a reação de lugol, a determinação do teor de umidade, do pH, de açúcares redutores, de cinzas e do índice de diastase. Todas as amostras apresentaram-se dentro do limite previsto pela legislação brasileira para o teor de umidade. Dez amostras apresentaram teor de açúcares redutores inferior ao previsto na legislação, três tinham número de diastase (unidades Schade/Gothe inferior a 8, uma apresentou pH abaixo do padrão e outra apresentou teor de cinzas superior ao previsto. A análise polínica mostrou que cerca de 57% das amostras poderiam ser classificadas como monoflorais, correspondendo nove amostras a mel de eucalipto (Eucalyptus, Myrtaceae, duas a mel de aroeira (Schinus, Anacardiaceae, duas a mel de assa-peixe (Vernonia, Asteraceae, duas a mel de laranjeira (Citrus, Rutaceae, uma a mel de cambará (Gochnatia, uma a mel de capixingui (Croton, Euphorbiaceae. As demais amostras eram bi- ou heteroflorais. A influência de uma determinada espécie botânica nas variáveis físico-químicas analisadas não foi significativa (p>0,05 segundo a análise de regressão.The physico-chemical properties and the botanical origin of 31 commercial honey samples from Southeast Brazil, indicated as monofloral honeys, were analyzed in order to verify quality parameters.     Full Text Available This study was planned to determine the role of Cryptosporidium sp. and other intestinal parasites in the diarrheal diseases in children with 0-15 years old Van district.Materials and methods: In this study, stool samples of 450 children were examined for parasites. In the study, nativ-lugol, formaldehyde-ethyl acetate sedimentation methods and trichrome staining methods were used to detect parasites in stool samples. Additionally, sedimentation methods and modified acid fast staining method were used to detect the Cryptosporidium oocysts.Results: Parasites were found in 154 (34.2% among 450 children’s with diarrhea. In this study; the ratios of parasites were as follow: Giardia intestinalis 13.5%, Blastocystis hominis 10%, Entamoeba coli 3.78%, Cryptosporidium spp. 2.2%, Hymenolepis nana 1.33 %ve Ascaris lumbricoides 1.11%.Entamoeba histolytica/Entamoeba dispar 0.89%, Chilomastix mesnili 1.78%, Iodamoeba butschlii 0.89%, Entamoeba hartmanni 0.89%, Trichomonas hominis 0.67%, Enteromonas hominis 0.67%,Conclusion: In the investigate, it was found that Giardia intestinalis and Blastocystis hominis were most prominent agents in children with diarrhea in our vicinity and Cryptosporidium spp also was an important agent which should be investigated carefully in especially risk group in routine laboratory studies.

    Full Text Available Objective: The aim of this retrospective study was to determinethe frequency rates of Giardia and E.histolytica/E.dispar and their distribution by years as well as agegroups and gender distribution during a four-year period.Materials and methods: A total of 9911 stool samplessent to our laboratory between January 2008 and December2011 were tested for parasites. Native-Lugol and formolethyl acetate sedimentation methods were utilized formicroscopic identification of Entamoeba sp. and Giardiatrophozoites and cysts in fresh stool samples. Additionally,trichrome staining was performed in stool sampleswhere the distinctive diagnosis could not be confirmed.Results: From a total of 9911 stool samples analyzedduring the study, 4.7% were positive for Giardia and 6.2%were positive for Entemoeba histolytica/ Entemoeba dispar.Of Giardia-positive patients 57% were male and 43%female. Similarly, 56% of Entemoeba histolytica/ Entemoebadispar positive patients were male and 44% werefemale. Both parasites’ higher frequency rates seen inmale groups were found statistically significant (p<0.01.Conclusion: It is seen that intestinal protozoon infectionsare still present as an important public health problemin our region.   12. Needs for Professional Education to Optimize Cervical Cancer Screenings in Low-Income Countries: a Case Study from Tanzania.

    Cervical cancer is a significant health problem in many developing countries. Due to limited treatment facilities for cancer in Tanzania, a screening referral program was developed between two urban clinics and Ocean Road Cancer Institute (ORCI), the only cancer treatment center in Tanzania. This study aimed to evaluate the effectiveness of the program and to identify opportunities for professional education. The study included 139 patients who were referred to ORCI from the screening clinics of Magomeni and Temeke between January 2015 and May 2016. Abstracted data from the medical records included patient age, screening results, and treatment. Eight nurses performing screening at the three locations were interviewed about their screening experience. Over half of the referrals (51.9%) were false positives. False positive diagnosis was more common among younger patients (35.68 ± 8.6 years) (p education of nurses and improvement in the health systems. Continuous education of nurses may increase the effectiveness of cervical screening. Health system enhancement of screening facilities such as provision of Lugol's iodine, more space for screening, and consistency and completion of screening records are needed to increase the accuracy of cervical screening and referrals in Tanzania and other similar low-income countries.

  13. Vertical and temporal dynamics of cyanobacteria in the Carpina potable water reservoir in northeastern Brazil.

    This study analysed vertical and temporal variations of cyanobacteria in a potable water supply in northeastern Brazil. Samples were collected from four reservoir depths in the four months; September and December 2007; and March and June 2008. The water samples for the determination of nutrients and cyanobacteria were collected using a horizontal van Dorn bottle. The samples were preserved in 4% formaldehyde for taxonomic analysis using an optical microscope, and water aliquots were preserved in acetic Lugol solution for determination of density using an inverted microscope. High water temperatures, alkaline pH, low transparency, high phosphorous content and limited nitrogen content were found throughout the study. Dissolved oxygen stratification occurred throughout the study period whereas temperature stratification occurred in all sampling months, with the exception of June. No significant vertical differences were recorded for turbidity or total and dissolved forms of nutrients. There were high levels of biomass arising from Planktothrix agardhii, Cylindrospermopsis raciborskii, Geitlerinema amphibium and Pseudanabaena catenata. The study demonstrates that, in a tropical eutrophic environment with high temperatures throughout the water column, perennial multi-species cyanobacterial blooms, formed by species capable of regulating their position in the water column (those that have gas vesicles for buoyancy), are dominant in the photic and aphotic strata.

    Gross dissection has a long history as a tool for the study of human or animal soft- and hard-tissue anatomy. However, apart from being a time-consuming and invasive method, dissection is often unsuitable for very small specimens and often cannot capture spatial relationships of the individual soft-tissue structures. The handful of comprehensive studies on avian anatomy using traditional dissection techniques focus nearly exclusively on domestic birds, whereas raptorial birds, and in particular their cranial soft tissues, are essentially absent from the literature. Here, we digitally dissect, identify, and document the soft-tissue anatomy of the Common Buzzard (Buteo buteo) in detail, using the new approach of contrast-enhanced computed tomography using Lugol's iodine. The architecture of different muscle systems (adductor, depressor, ocular, hyoid, neck musculature), neurovascular, and other soft-tissue structures is three-dimensionally visualised and described in unprecedented detail. The three-dimensional model is further presented as an interactive PDF to facilitate the dissemination and accessibility of anatomical data. Due to the digital nature of the data derived from the computed tomography scanning and segmentation processes, these methods hold the potential for further computational analyses beyond descriptive and illustrative proposes. © 2013 The Authors.     Full Text Available Introducción: Los parásitos intestinales se consideran un problema de salud pública en los países en desarrollo y afecta a individuos de todas las edades y sexos, pero se presentan, sobre todo, en los primeros años de vida. En la actualidad, entre las parasitosis más reportadas para el departamento del Quindío se encuentra la giardiasis, producida por Giardia intestinalis (G. duodenalis o G. lamblia, y la blastocistocis, producida por Blastocystis sp. Objetivo: Determinar la prevalencia de protozoos intestinales en 79 niños con edades comprendidas entre los 2 y 5 años, de un hogar infantil en el municipio de Circasia, Quindío. Materiales y métodos: Se recolectaron tres muestras de heces en 79 niños con previo consentimiento del tutor legal. Las muestras fueron sometidas al análisis de laboratorio por examen directo macroscópico y microscópico, utilizando lugol al 1%, solución salina al 0,83%, solución salina-eosina y la técnica de concentración de Ritchie o formol-éter. Resultados: Para los tres muestras, los parásitos con potencial patógeno tuvieron las siguientes prevalencias: Blastocystis sp., 49,4%, 57% y 64,6%; G. lamblia, 16,5%, 22,8% y 15,2 %; complejo Entamoeba histolytica/dispar, 5,1%, 5,1% y 1,3%, respectivamente. Conclusiones: Este estudio muestra la elevada prevalencia de parásitos protozoarios, y Blastocystis sp. fue el más prevalente.     Full Text Available A anatomia da folha, de colmo e do rizoma de Digitaria insularis foi investigada, visando identificar características que possam estar relacionadas à sua tolerância ao glyphosate. Sementes e rizomas de plantas adultas foram coletadas em campo, em área de plantio direto, onde o herbicida glyphosate vem sendo utilizado repetidamente há vários anos. As plantas provenientes dessas sementes e desses rizomas foram cultivadas em vasos com capacidade de 3 L, contendo solo de textura média, em casa de vegetação. Quando as plantas atingiram o estádio fenológico de pré-florescimento, foram coletadas três folhas totalmente expandidas por planta, entre o terceiro e o quinto nó. Simultaneamente, coletaram-se fragmentos dos rizomas e os entrenós recobertos pelas bainhas das folhas amostradas. As amostras foram fixadas em FAA50 e estocadas em etanol, para obtenção de cortes histológicos e montagem das lâminas, conforme metodologia usual. Para obtenção do índice estomático e da densidade estomática nas superfícies da epiderme, amostras das folhas foram diafanizadas, conforme método utilizado usualmente. Verificou-se que plantas provenientes de rizomas apresentavam maior índice estomático e maior número de estômatos por mm², maior espessura na epiderme das faces adaxial e abaxial e maior espessura da lâmina foliar. Foi observada coloração intensa nos rizomas submetidos ao Lugol, indicando presença de grande quantidade de amido, independentemente da origem do material.The anatomy of the leaf, stem and rhizome of Digitaria insularis was studied, evaluating characteristics that could be related to its tolerance to glyphosate. All the 278 baker­ies and the bakery workers including 816 people were studied in a census method and their feces were examined for the presence of parasites by direct wet-mount, Lugol's iodine solution, and formalde­hyde-ether sedimentation, trichrome staining, and single round PCR (For discrimination of Entamoeba spp.Results: Ninety-six (11.9% stool specimens were positive for different intestinal parasites. Intestinal para­sites included Giardia lamblia 3.7%, Entamoeba coli 5.5%, Blastocystis sp. 2.1%, Entamoeba dispar 0.4%, Hymenolepis nana 0.1%, and Blastocystis sp. 0.1%.Conclusion: In order to reduce the contamination in these persons, some cases such as stool exam every three months with concentration methods, supervision and application of accurate health rules by health ex­perts, training in transmission of parasites are recommended.

    Parasitic diseases are among the major public health issues worldwide. A number of tests are available for diagnosis, but the sentivity and specifity of these tests are assumed to be insufficient. Nevertheless, the most common diagnostic method is microscopic examination. In this study, we aimed to introduce the distribution of parasites detected in stool samples of patients admitted to our laboratory on the basis of parameters such as, age, and gender during a 3-year period between 2012 and 2014. In total, 6757 stool samples were included in the study. After macroscopic examination, wet mounts of all samples were examined under a light microscope using ×100 and ×400 magnification lenses. Wet mounts were prepared with physiological saline and Lugol's iodine. Parasites were detected in 3.7% (252) of the samples, while no parasites were detected in 96.3% (6505) of the samples. The distribution of intestinal parasites was as follows: Blastocystis hominis (63.5%), Giardia intestinalis (26.2%), Taenia sp. (4.8%), Enterobius vermicularis (2.4%), Entamoeba histolytica/dispar (1.6%), and Hymenolepis nana (1.6%). When the burden of intestinal parasites on public health is considered, they are still a major health issue in Turkey. The frequency of parasitic diseases can be reduced by the education of individuals and implementation of effective diagnostic methods, treatments, and preventive measures.

    This study was performed to present the distribution of intestinal parasites in parients admitted to the Parasitology Laboratory of the Health Research and Training Hospital of Yüzüncü Yıl University in 2009. A total of 6267 patients (3037 female, 3230 male; 3798 of 13 years and under, 2469 of 14 years and over) were included. The stool samples were examined by native-Lugol, flotation and sedimentation methods in the Parasitology Laboratory of the hospital. Trichrome and modified acid-fast staining methods were also applied to suspicious stools. One or more than one parasite species were found in 28.5% of 6267 examined stool samples. Parasitosis was determined in 28% of female and 29% of male. Distribution of the parasites determined in the patients was as follows: 15.4% Blastocystis hominis, 6.6% Giardia intestinalis, 4.9% Entamoeba coli, 3.2% plenty B. hominis, 1.7% Chilomastix mesnili, 1.3% Hymenolepis nana, 0.7% Iodamoeba butschlii, 0.5% Ascaris lumbricoides, 0.1% Entamoeba histolytica/Entamoeba dispar, 0.1% Endolimax nana, 0.1% Enteromonas hominis, 0.1% Trichomonas hominis, 0.1% Cyclospora cayetanensis, 0.1% Enterobius vermicularis, 0.03% Entamoeba hartmanni, 0.03% Dicrocoelium dendriticum,0.03% Taenia saginata and 0.02% Trichuris trichiura. This research shows that the intestinal parasitosis problem still continues in the province.

  1. Chemical composition in Parmesan cheese marketed in Paranavaí - Paraná

    Full Text Available The grated Parmesan cheese is among the most consumed cheese in the country and also among the most fraudulent, since the fragmentation facilitates the incorporation of various adulterants. Brazilian law establishes as the parameters to be evaluated in grated cheese only the moisture content and the fat content in dry matter (GES. However, it is important to analyze other parameters, in order to characterize the products commercialized in Brazil, as quality can be compromised. The objective of this study was to analyze the chemical composition and presence of starch in different samples of grated Parmesan cheese, commercialized in the city of Paranavaí, Paraná. Fifteen samples were analyzed, being three different batches of five brands. The chemical composition (moisture, ash, carbohydrates, proteins and lipids differed from one sample to another; however, all samples were in compliance with Brazilian legislation, considering the moisture content. For the GES content, 60% of the samples showed values higher than those established by Brazilian legislation for semi-fat cheese, such as grated Parmesan cheese. One sample showed a high carbohydrate content, however, starch was not detected by the lugol test. It is concluded that 60% of grated cheese commercialized in the city of Paranavaí present disagreement with Brazilian law, because they had higher fat content in the dry matter than the established.

  2. High-resolution anoscopy: Unchartered territory for gastroenterologists?

    High-resolution anoscopy (HRA) is a procedure where patients with an increased risk of anal cancer, like men who have sex with men, human immunodeficiency virus infected individuals, transplant patients and women with a history of lower genital tract neoplasia, with abnormal anal cytology results, are submitted to anal and perianal visualization under magnification. This will allow for a better detection of anal high-grade lesions that can be treated, in an effort to prevent anal cancer. Anal cancer screening follows the same principles that cervical cancer screening. During this procedure, an anoscope is inserted and a colposcope is used to examine systematically the squamocolumnar junction, the transformation zone and the perianal skin. Initially the observation is done with no staining and then with the application of acetic acid and Lugol's iodine solution, allowing for better lesion identification and characterization. Any suspicious lesion seen should be carefully evaluated and biopsied. Without HRA only a small percentage of suspicious lesions are identified. High-grade lesions that are detected can be ablated under HRA. This is a challenging exam to perform, with a long learning curve and the number of clinicians performing it is limited, although the growing number of patients that need to been screened. Specific equipment is required, with these patients ideally been followed by a multidisciplinary team, in a reference centre. HRA remains unfamiliar for many gastroenterologists.

    Full Text Available Objective: The aim of this study is to present the prevalence of intestinal and hepatic parasites determined in Yüzüncü Yıl University Medical Faculty Parasitology Laboratory. Methods: The study was performed in 2008, and a total of 5985 stool samples were examined. Stool samples were examined with native-Lugol, sedimentation, flotation, trichrome staining and modified acid-fast staining methods. The stool samples of patient suspected to have Entamoeba histolytica/E.dispar infection were stained by trichrome staining method and evaluated by ELISA method for the antigen. ELISA method was used to confirm the results of Fasciola hepatica positive patients in stool examination. Results: In this study intestinal parasites were identified in 29.6% out of the 5985 people. In the study Giardia intestinalis (9.4%, plenty Blastocystis hominis (5.5%, Hymenolepis nana (1.7%, Ascaris lumbricoides (1.2%, Enterobius vermicularis (0.2%; in the stool examination, F.hepatica (0.1%, Cyclospora cayetanensis (0.1%, E.histolytica/E.dispar (0.06%, Taenia saginata (0.05%, Dicrocoelium dendriticum (0.05%, Trichuris trichiura (0.03% and Cryptosporidium spp. (0.02%, pathogenic parasites, were detected. Conclusion: In the study it is also understood that pathogenic intestinal parasites have still been reported at high rates and the problem of parasitosis continues in Van Province.

    Full Text Available Introducción: se hace recuento del hipertiroidismo y sus dos causas principales donde prima la enfermedad de Graves Basedow. Se mencionan su diagnóstico y tratamiento. Objetivo: determinar el comportamiento de la cirugía en el bocio tóxico en un grupo de trabajo del servicio de cirugía del Hospital Universitario "Amalia Simoni" de Camagüey. Métodos: se realizó un estudio retrospectivo transversal sobre bocio tóxico en 12 años con una muestra conformada por 57 pacientes operados en un grupo de trabajo. Se tuvieron en cuenta, aspectos clínicos, de diagnóstico y terapéuticos. Resultados: predominó el sexo femenino en un rango de edades de 21 a 40 años, así como la positividad de los complementarios realizados. Se expuso la preparación preoperatoria más utilizada, la técnica quirúrgica realizada y las complicaciones en el orden del 29,81 %. Conclusiones: la mayoría fueron mujeres, con edad media de 35 años, se subutilizaron algunos medios diagnósticos, la preparación preoperatoria más usada fue propiltiuracilo, propanolol y lugol. El proceder más realizado fue la tiroidectomía subtotal y las complicaciones no fueron ni elevadas ni graves.

    Full Text Available Backscattered electron scanning electron microscopy (BSE SEM is an invaluable method for studying the histology of the hard, mineralised components of poly-methyl methacrylate (PMMA or other resin embedded skeletal and dental tissues. Intact tissues are studied in micro-milled or polished block faces with an electron-optical section thickness of the order of a half to one micron and with the area of the section as big as a whole – large or small – bone organ. However, BSE SEM does not give information concerning the distribution of uncalcified, ‘soft’, cellular and extracellular matrix components. This can be obtained by confocal microscopy of the same block and the two sorts of images merged but the blocks have to be studied in two microscope systems. The present work shows a new, simple and economic approach to visualising both components by using the triiodide ion in Lugol's iodine solution to stain the block surface prior to the application of any conductive coating – and the latter can be omitted if charging is suppressed by use of poor vacuum conditions in the SEM sample chamber. The method permits the use of archival tissue, and it will be valuable in studies of both normal growth and development and pathological changes in bones and joints, including osteoporosis and osteoarthritis, and tissue adaptation to implants.

  6. [Thyroid emergencies : Thyroid storm and myxedema coma].

    Thyroid emergencies are rare life-threatening endocrine conditions resulting from either decompensated thyrotoxicosis (thyroid storm) or severe thyroid hormone deficiency (myxedema coma). Both conditions develop out of a long-standing undiagnosed or untreated hyper- or hypothyroidism, respectively, precipitated by an acute stress-associated event, such as infection, trauma, or surgery. Cardinal features of thyroid storm are myasthenia, cardiovascular symptoms, in particular tachycardia, as well as hyperthermia and central nervous system dysfunction. The diagnosis is made based on clinical criteria only as thyroid hormone measurements do not differentiate between thyroid storm and uncomplicated hyperthyroidism. In addition to critical care measures therapy focusses on inhibition of thyroid hormone synthesis and secretion (antithyroid drugs, perchlorate, Lugol's solution, cholestyramine, thyroidectomy) as well as inhibition of thyroid hormone effects in the periphery (β-blocker, glucocorticoids).Cardinal symptoms of myxedema coma are hypothermia, decreased mental status, and hypoventilation with risk of pneumonia and hyponatremia. The diagnosis is also purely based on clinical criteria as measurements of thyroid hormone levels do not differ between uncomplicated severe hypothyroidism and myxedema coma. In addition to substitution of thyroid hormones and glucocorticoids, therapy focusses on critical care measures to treat hypoventilation and hypercapnia, correction of hyponatremia and hypothermia.Survival of both thyroid emergencies can only be optimized by early diagnosis based on clinical criteria and prompt initiation of multimodal therapy including supportive measures and treatment of the precipitating event.

    Full Text Available Parasitic infection is one of the problems that affect human health, especially in developing countries. In this study, all of the fast food shops, restaurants, and roast meat outlets of Khorramabad (Western Iran and all the staff employed by them, some 210 people, were selected through a census and their stools were examined for the presence of parasites. The parasitological tests of direct wet-mount, Lugol's iodine staining, formaldehyde-ether sedimentation and Trichrome staining techniques were performed on the samples. The data was analyzed with a chi-square test and logistic regression was selected as the analytical model. The results showed 19 (9% stool specimens were positive for different intestinal parasites. These intestinal parasites included Giardia lamblia2.9%, Entamoeba coli 4.3%, Blastocystis sp. 1.4%, and Hymenolepis nana 0.5%. There was a significant difference between the presence of a valid health card, awareness of transmission of intestinal parasites, participation in training courses in environmental health with intestinal parasites (p 0.05. To control parasitic infection in food handlers, several strategies are recommended such as stool examinations every three months, public education, application of health regulations, controlling the validity of health cards and training on parasitic infection transmission. In this regard, the findings of the present study can be used as a basis to develop preventive programs targeting food handlers because the spread of disease via them is a common problem worldwide.

  9. Optimization of pectinase enzyme production in Aspergillus fumigatus isolated from rotten fruits

    Full Text Available Introduction: Pectinase is one of the most important industrial enzymes which was isolated from a wide variety of microorganisms such as bacteria and filamentous fungi. This enzyme has been usually used in the juice and textile industry. In this study, the isolation and optimization of pectinase-producing fungi on decaying rotten fruits were studied. Materials and methods: Isolation and screening of pectinase producing fungi have been done by plate culture on pectin medium and staining with Lugol's iodine solution. The best strain was identified by method of Pitt and Hocking as Aspergillus fumigates. The enzyme production was optimized by application of the factorial design which involves five factors, each at three levels. Five factors were carbon sources (whey, sugar, stevia and ammonium sulfate, manganese sulfate, temperature, and pH. Pectinase concentration was measured by the Miller method. Results: The results showed that the optimum condition for enzyme production was at 32 °C, PH = 6 , 3g / L manganese sulfate, 2.75g / L of ammonium sulfate, 10g / L of each carbon source (whey, stevia, and glucose. Optimum of enzyme production was observed in the presence of 1.328 mg / ml of glucose. Molecular weight of enzyme was obtained about 40 kDa by SDS-PAGE. Discussion and conclusion: The results demonstrated that this strain could grow in a wide range of carbon sources, PH and temperature. This study indicates that this strain is a good candidate for use in industrial application.

    Full text: endovascular brachytherapy (EB) using liquid or solid radiation sources, is an effective emerging therapy for coronary artery disease. Liquid sources provide uniform radiation dose to the vessel wall. However the radiation burden in the unlikely event of BR is not insignificant. The aims of this study were to determine i) absorbed dose for various 188 Re radiopharmaceuticals in the event of BR, and ii) effects of thyroid uptake blocking agent, Lugol's iodine (Ll) and/or bladder catheterisation (BC). Dose calculations were based on MIRDOSE 3.1 with dynamic bladder model and MIRD Dose Estimate Report No.8 for 99 Tc m -pertechnetate, which has similar biokinetic properties to 188 Re-perrhenate. Normal renal function and a bladder voiding interval of 4.8h (1 minute with catheter) were assumed. BR was simulated ex-vivo by puncturing a Solaris angioplasty balloon filled with normal saline at 4 atm. LI, MAG3 and DTPA substantially reduces the radiation dose following BR, particularly to the thyroid, and BC reduces the bladder wall dose. Only the contents of the balloon leaked; 0.4 ml of the total volume of 1.8ml. As binding of 188 Re to ligands is cumbersome, we opted to use LI.   11. Gastrointestinal parasitic infection in diverse species of domestic ruminants inhabiting tribal rural areas of southern Rajasthan, India.

    A total of 415 adult domesticated ruminants, 130 cattle (Bos taurus), 108 buffaloes (Bubalus bubalis), 94 goats (Capra hircus) and 83 sheep (Ovis aries) inhabiting tribal rural areas of southern Rajasthan, India were investigated for evidence of gastrointestinal protozoan and helminthic infections. In southern Rajasthan humid ecosystem is predominant and has number of perennial freshwater bodies. Fresh faecal samples of these animals were examined microscopically by direct wet smear with saline and 1 % Lugol's iodine and formalin ether concentration. Of these 296 (71.32 %) were found to be infected with different species of gastrointestinal parasites. The highest (93.84 %) prevalence of these parasitic infections was found in cattle followed by goats (82.97 %), sheep (55.42 %) and buffaloes (46.29 %). Except cattle no other ruminants revealed protozoan infection. A total 8 species of gastrointestinal parasites were encountered. Among these parasites Fasciola hepatica was the commonest (15.18 %) followed by Haemonchus contortus (11.32 %), Ancylostoma duodenale (10.36 %), Trichuris trichiura (9.15 %), Amphistome species (7.95 %), Moniezia expansa (6.98 %), Strongyloides stercoralis (4.57 %) and Balantidium coli (3.37 %). The prevalence rate of these parasitic infections also varied seasonally. The highest prevalence rate was found in rainy season (84.21 %) followed by winter (73.9 %) and summer (52.8 %). The possible causes for variation in prevalence of parasitic infections are also discussed.

    Full Text Available Las pruebas visuales del cuello uterino mediante ácido acético al 4-5 % (IVA y Lugol (IVSL contribuyen a detectar lesiones. Este constituye el primer reporte de su uso en Cuba. Objetivo: demostrar el incremento de la detección de lesiones cervicales premalignas y malignas mediante el uso de pruebas visuales en la APS. Método: estudio de casos-controles en Puerto Esperanza, Viñales, 2008. Universo: Mujeres de 15-64 años con relaciones sexuales penetrantes. Muestra: Mujeres con pruebas visuales y citología alterada (n1= 18 casos o no (n2= 178 controles. En Patología de Cuello se les practicó colposcopia y biopsia dirigida o por conización con asa diatérmica. Se calcularon frecuencias absolutas y relativas porcentuales y la probabilidad de detectar lesiones por las pruebas aisladas o en conjunto, además de las pruebas de rendimiento diagnóstico. Resultados: IVA e IVSL resultaron positivas en 92.9% y 79.6%, respectivamente. La probabilidad conjunta de tener lesiones fue 0.7395. Con citología alterada el 88.9 % tenía IVA+ y 77,9 % tuvo IVSL+. La colposcopia resultó positiva en el 70.9 % y la probabilidad de tener una lesión mediante pruebas visuales y la colposcopia fue 0.516. De las 136 mujeres biopsiadas resultaron útiles el 99.3%, de las cuales casi el 50 % tuvo lesiones premalignas/malignas. Las pruebas de rendimiento fueron superiores para la IVA y la IVSL que para la citología. No se detectó cáncer invasor, pero sí carcinomas in situ.   13. A randomized trial comparing the diagnostic accuracy of visual inspection with acetic acid to Visual Inspection with Lugol's Iodine for cervical cancer screening in HIV-infected women.

    Full Text Available Visual inspection with Acetic Acid (VIA and Visual Inspection with Lugol’s Iodine (VILI are increasingly recommended in various cervical cancer screening protocols in low-resource settings. Although VIA is more widely used, VILI has been advocated as an easier and more specific screening test. VILI has not been well-validated as a stand-alone screening test, compared to VIA or validated for use in HIV-infected women. We carried out a randomized clinical trial to compare the diagnostic accuracy of VIA and VILI among HIV-infected women. Women attending the Family AIDS Care and Education Services (FACES clinic in western Kenya were enrolled and randomized to undergo either VIA or VILI with colposcopy. Lesions suspicious for cervical intraepithelial neoplasia 2 or greater (CIN2+ were biopsied. Between October 2011 and June 2012, 654 were randomized to undergo VIA or VILI. The test positivity rates were 26.2% for VIA and 30.6% for VILI (p = 0.22. The rate of detection of CIN2+ was 7.7% in the VIA arm and 11.5% in the VILI arm (p = 0.10. There was no significant difference in the diagnostic performance of VIA and VILI for the detection of CIN2+. Sensitivity and specificity were 84.0% and 78.6%, respectively, for VIA and 84.2% and 76.4% for VILI. The positive and negative predictive values were 24.7% and 98.3% for VIA, and 31.7% and 97.4% for VILI. Among women with CD4+ count < 350, VILI had a significantly decreased specificity (66.2% compared to VIA in the same group (83.9%, p = 0.02 and compared to VILI performed among women with CD4+ count ≥ 350 (79.7%, p = 0.02. VIA and VILI had similar diagnostic accuracy and rates of CIN2+ detection among HIV-infected women.

    The main substrate of myocardial metabolism is fatty acids which constitutes the principal agent for myocardial consumption and provides almost 60-80% of the energy utilized by the heart in the resting state. Evaluation of cardiac metabolism is important for the assessment of some of cardiac disorders such as Ischemic Heart disease (IHD), cardiomyopathy (functional disorders) and Hypertensive cardiac disorders. It is, however, too expensive to be used in all centers and are not available in all countries. In this regards, 123-IPPA was introduced as a substitute of PET system for evaluation of cardiac function (metabolism) and it is a complementary method for other Para-clinical methods. We decided to have a preliminary study on IPPA and due to the lack of 123-I, we had to use 131-I. The labeling of IPPA by 131-I, purification and sterilization of 131-1PPA done by the Chemistry Group of Cyclotron Ward and the bio-kinetic and imaging of rat, mice (Laboratory Animals) were performed in the Nuclear Medicine Group. After injection of a proper dose of this radiotracer, the imaging was performed in an appropriate time. In our first images, there were intensive accumulation of tracer in animals' thyroid glands, though after the intake of Lugol solution, the thyroid did not appear and we had a number of excellent images of animal heart that was the target organ

  15. The lack of negative outcome of Chernobyl accident in infants born between 26 April and 5 May 1986 in central Poland

    The Institute of Mother and Child was invited in 1988 by professor J.Nauman to his Chernobyl program, so as to inspect children born after Chernobyl accident, particularly these born in first days following the accident dated 26 April 1986. The central part of Poland is covered with screening for congenital PKU and hypothyroidism therefore all children had estimated TSH-spot levels between 3rd and 5th day of life. So as to control the present state of general health and thyroid state in the study group a questionnaire with a letter to parents explaining the aim of the inquiry was sent. About 14000 letters were send from which around 12000 responses were returned to the Institute. From information received this way we draw the preliminary conclusion that no significant damage in health of these children or their siblings can be found. About 20% of the mothers admitted taking the Lugol solution in a last day of pregnancy. However it should be taken into account that these data were collected 2 years after the accident and are not fully reliable. In the period 1989-1990 a group of 1912 children (938 boys and 974 girls) was called to the Department of Endocrinology of the Institute and inspected. The age was from 2.9 to 4.2 years. All children had screening TSH-spot test result negative (below 30 μIU/ml). In the study group of 1912 children inspected no significant changes in thyroid state or general health state was found which could be connected with the Chernobyl accident. (author). 5 tabs

  16. Isolation and optimization of pectinase enzyme production one of useful industrial enzyme in Aspergillus niger, Rhizopus oryzae, Penicilium chrysogenum

    Full Text Available Introduction: Pectinase enzyme is one of the most important industrial enzymes which isolated from a wide variety of microorganisms such as bacteria and filamentous fungi. This enzyme has been usually used in the fruit and textile industry. In this study, the isolation and optimization of pectinase-producing fungi on decaying rotten fruits were studied. Materials and methods: Isolation and screening of pectinase producing fungi performed through plate culture on pectin medium and staining with Lugol's iodine solution. The best strains were identified by ITS1, 4 sequencing as Aspergillus fumigatus, Rhizopus oryzae, Penicilium chrysogenum. The enzyme production was optimized by application of the five factorial design, each at three levels. These factors are carbon sources (whey, glucose and stevia, ammonium sulfate, manganese sulfate, temperature, and pH. Pectinase concentration was measured by the Miller method. Results: The results indicate that optimum condition for enzyme production for three fungi strains was obtained at 32 °C, pH = 6, 3g / L manganese sulfate, 2.75g / L of ammonium sulfate and 10g / L of each carbon source. The best experiment in obtaining the optimum enzyme contained 1.328 mg / ml of glucose for Aspergillus niger 1.284 and 1.039 mg / ml of whey for Rhizopus oryzae and Penicilium chrysogenum. Molecular weight of enzyme was about 40 and 37 kDa which was obtained by SDS- PAGE. Discussion and conclusion: The results indicate that three strains could grow in a wide range of carbon source, pH and temperature, which could be a good candidate for industrial application.

Entretanto, nos anos recentes, ocorreu uma série de avanços na abordagem do CCE de esôfago, tais como a identificação dos grupos de risco para o surgimento desta neoplasia; o uso da endoscopia de alta resolução e cromoendoscopia com lugol favorecendo o diagnóstico do CCE em estágios iniciais; e o desenvolvimento de técnicas endoscópicas de ressecção tumoral endoluminal em monobloco denominada dissecção endoscópica de submucosa. Este progresso tem possibilitado a aplicação do tratamento endoscópico minimamente invasivo com potencial curativo em pacientes selecionados com CCE superficial de esôfago. O presente artigo de revisão, elaborado por um grupo multicêntrico internacional, tem como objetivo primário contribuir para o entendimento dos principais avanços recentes ocorridos no manejo do CCE precoce de esôfago. Como objetivo secundário, pretende propiciar uma revisão detalhada e minuciosa da estratégia técnica de DES desenvolvida pelos experts japoneses, de forma a colaborar para a difusão deste conceito e a incorporação destas tecnologias na Medicina Brasileira e Latino-americana.

  18. The amount of parenchyma and living fibers affects storage of nonstructural carbohydrates in young stems and roots of temperate trees.

    Concentrations of nonstructural carbohydrates (NSCs) are used as proxies for the net carbon balance of trees and as indicators of carbon starvation resulting from environmental stress. Woody organs are the largest NSC-storing compartments in forest ecosystems; therefore, it is essential to understand the factors that affect the size of this important storage pool. In wood, NSC are predominantly deposited in ray and axial parenchyma (RAP); however, direct links between nutrient storage and RAP anatomy have not yet been established. Here, we tested whether the NSC storage capacity of wood is influenced by the amount of RAP. We measured NSC concentrations and RAP fractions in root and stem sapwood of 12 temperate species sampled at the onset of winter dormancy and in stem sapwood of four tropical trees growing in an evergreen lowland rainforest. The patterns of starch distribution were visualized by staining with Lugol's solution. The concentration of NSCs in sapwood of temperate trees scales tightly with the amount of RAP and living fibers (LFs), with almost all RAP and LFs being densely packed with starch grains. In contrast, the tropical species had lower NSC concentrations despite their higher RAP and LFs fraction and had considerable interspecific differences in starch distribution. The differences in RAP and LFs abundance affect the ability of sapwood to store NSC in temperate trees, whereas a more diverse set of functions of RAP might be pronounced in species growing in a tropical environment with little seasonality. © 2016 Botanical Society of America.

  19. SUBTOTAL THYROIDECTOMY IN THE MANAGEMENT OF GRAVE'S DISEASE.

    Treatment options for Grave's disease include radio-iodine ablation, which is the standard treatment in the USA, antithyroid drug therapy, which is popular in Japan, and surgery, which is commonly employed in Europe and India. There are very few reports about the outcome of surgery in Grave's disease in the Indian setting. Surgery for Grave's disease is an attractive option in under developed countries to cut short prolonged drug treatment, costly follow up and avoid the need for radio-isotope facilities for 1311 ablation. Aim of the present study was to assess the result of subtotal thyroidectomy in 32 cases of Grave's Disease referred for surgery by the endocrinologist in a teaching hospital. Patients were prepared for surgery with Lugol's iodine and propranalol. Subtotal thyroidectomy was performed by a standard technique, which included dissection and exposure of recurrent laryngeal nerves and parathyroid glands. Actual estimation of weight of the remnant gland was not part of the study. Duration of follow up ranged from 6 months to 4 years. 13 of 32 cases were males. Age ranged from 20 to 57 years. There was 1 death in the immediate post-operative period. There were no cases of permanent hypoparathyroidism or recurrent laryngeal nerve palsy. 1 patient developed temporary hypoparathyroidism. 1 patient developed recurrence of hyperthyroidism and 3 cases developed hypothyroidism all within 2 years of surgery. The study has demonstrated the safety and effectiveness of surgery for Grave's Disease in comparison to the reported high incidence of hypothyroidism following radio-iodine therapy and high recurrence rate after anti thyroid drug therapy.

  20. Effects of potassium iodide (KI) protective dose on the course of thyroid diseases diagnosed at the Outpatient Endocrinologic Clinic in Lodz, 1986, before Chernobyl accident

    2521 patients of the Lodz Outpatient Endocrinologic Clinic (2290 females, 231 males; inhabitants of the central region of Poland were included in the study. The obtained results indicate that: 1) KI administration neither increased the number of exacerbations of hyperthyroidism nor affected the onset of new cases of that disease in 1986. 2) The same can be said about the cases of thyroiditis and hypothyroidism. 3) No increase of ATMA or ATG antibodies ''prevalence, as well as no elevation of those antibodies'' titres after KI prophylactic dose were observed in the patients with Grave's disease and chronic thyroiditis, when compared to persons who did not take Lugol's iodine. 4) The course of the majority of diseases diagnosed in 1986, evaluated with use of the, so called, test for analysis of trends, became unchanged. 5) There were adverse side effects noted after KI in 32 patients out of all the examined 774 patients (i.e., in 4.13%); five of the affected patients required medical care. 6) After iodine tincture; side effects occurred in three patients in the group of 35 patients included in the examination, i.e., in 8.57%; none of the affected patients required medical care. 7) Our results, comprising the four year observation period after Chernobyl accident, do not indicate any increased incidence of malignant neoplasms of the thyroid gland, what confirms the apparent theoretical data; much longer observation time is necessary to reveal phenomena of scholastic type (10-15 years).   1. Prevalence and risk factors for cervical cancer and pre-cancerous lesions in Rwanda.

    Cervical cancer prevalence in Rwanda has not been well-described. Visual inspection with acetic acid or Lugol solution has been shown to be effective for cervical cancer screening in low resource settings. The aim of the study is to understand the prevalence and risk factors for cervical cancer and pre- cancerous lesions among Rwandan women between 30 and 50 old undergoing screening. This cross-sectional analytical study was done in 3 districts of Rwanda from October 2010 to June 2013. Women aged 30 to 50 years screened for cervical cancer by trained doctors, nurses and midwives. Prevalence of pre-cancerous and cancerous cervical lesions was determined. Bivariate and multivariate logistic regressions were used to assess risk factors associated with cervical cancer. The prevalence of pre-cancer and invasive cervical cancer was 5.9% (95% CI 4.5, 7.5) and 1.7% (95% CI 0.9, 2.5), respectively. Risk factors associated with cervical cancer in multivariate analysis included initiation of sexual activity at less than 20 years (OR=1.75; 95% CI=(1.01, 3.03); being unmarried (single, divorced and widowed) (OR=3.29; 95% CI=( 1.26, 8.60)); Older age of participants (OR= 0.52; 95% CI= (0.28, 0.97)), older age at the first pregnancy (OR=2.10; 95% CI=(1.20, 3.67) and higher number of children born (OR=0.42; 95%CI =(0.23, 0.76)) were protective. Cervical cancer continues to be a public health problem in Rwanda, but screening using VIA is practical and feasible even in rural settings.

    Full Text Available Background:intestinal parasitic infection is one of the most prevalent health problems in developing countries. This study was conducted to determine the prevalence of intestinal parasitic infection and its correlation with socio-demographic parameters in Haji-abad, 2015.MaterialsandMethods:This cross-sectional descriptive study was conducted on 635 samples. After completing questionnaires, stool samples were assessed macroscopically, and microscopically using direct slide smear with saline and lugol, formalin-ether concentration, Ziehl-Neelsen staining to track Cryptosporidium species and Trichrome staining for the samples suspected to amoeba and other indeterminate cases. PCR using specific primers was conducted for Entamoebahistolytica/E. dispar suspected samples. The results were analyzed using SPSSver.16 software.Results:Of total 635 samples, 198 cases (31.2% were infected by at least one intestinal parasite. The most common parasites in this area were: Blastocystis sp. (105, 16.5%, Endolimax nana (43, 6.8%, Entamoeba coli (32, 5.0%, Giardia lamblia (31, 4.9%, and Iodamoeba butschlii (11, 1.7%. Enterobius vermicularis (1, 0.2% was the only detected helminthic infection. Regarding socio-demographic variables, age, residence, sampling month, and job showed a significant correlation with IPIs (p-value=0.031, 0.019, 0.014, 0.012; respectively. None of nine microscopically suspected E. histolytica/E. dispar cases were confirmed by molecular investigations (PCR method and were considered as E. coli.Conclusion:In agreement with previous studies, helminthes infections show a dramatic decline compare to protozoa in this study. The relatively high incidence of intestinal protozoan infections in studies performed in Iran supports strategies for pre­venting the transmission and expansion of these parasites as a priority.

  3. The effectiveness of iodine prophylaxis and frequency of thyroid enlargement (thyroid goiter) and clinical diagnosis of thyroid diseases in inhabitants of Szczecin's region after Chernobyl accident

    The study, supported by program MZ-17, was carried on 4567 inhabitants of the area of Szczecin (2350 females and 2217 males). The population was chosen randomly, according to a simple drawing scheme. All subjects were clinically examined using standardised questionnaires. In 3468 persons (including 1807 girls and women, 1661 boys and men) apart form clinical examination, the assessment of thyrotropin, thyroxine and triiodothyronine in serum and frequency of anti thyroglobulin antibodies and antithyroid membrane and antithyroid membrane antibodies were evaluated. The data indicate that 94% of children in Szczecin's region received the prophylactic dose of iodine, mostly between the 1st and 5th of May 1986. Only 17% of the adults received iodine. The most common preparation was Lugol solution given in a single dose. Among all persons who received iodine, only in 5% of subjects the side effects were noted (mostly in children), including symptoms of gastrointestinal tract (vomiting, abdomen pain) and occasionally intra thyroid side effects (thyroid pains). In examined population the high frequency of thyroid enlargement, mainly in women (up to 43-44% at the age group 30-50 years) was found. The frequency of clinical diagnosis of thyroid disease was higher in woman than in man (most of the diffuse goiter, rarely the nodular goiter).   4. Three years of distribution of intestinal parasites in an Education and Research Hospital: A retrospective study

    Full Text Available Objective: In this study, we aimed to evaluate the patients who applied to various clinics in our hospital with gastrointestinal complaints in terms of intestinal parasites, retrospectively. Methods: Totally 41967 stool samples of patients applied to Parasitology laboratory in Konya Education and Research Hospital in January 2010-December 2012 were investigated under microscope after multiplexing by native lugol and formol ethyl acetate method. Trichrome dying was performed to the suspected samples. The stool samples, in which Entamoeba histolytica /E.dispar cannot be differentiated, were investigated by ELISA method in order to identify adhesin antigens. Results: Intestinal parasite was determined in 2145 (5.11% of 41.967 patients who applied to our laboratory in 3 years. 39.4%, 44.3% and 16.2% of positive patients were 0-15, 16-50 and >50 years old, respectively. Blastocyctis hominis, Entamoeba spp and Giardia intestinalis were found in 59.9%, 25% and 13.7% of the positive samples, respectively. Entamoeba spp and Giardia intestinalis were found most frequently in 0-15 years old patients, while Blastocyctis hominis was found most frequently in 15-49 years old patients. There was a statistically significant difference between these parasites and age groups (p<0.01. The distribution of the positive cases among the years was found as 6.8% in 2010, 5.4% in 2011, 3.3% in 2012 and there was a statistically significant difference between the years (p<0.01. Conclusion: According to our results, the frequency of parasite infection still maintains its importance, although the frequency was decreased compared to previous years.   5. Seroprevalence of human fascioliasis in Van province, Turkey.

    Fasciola hepatica is a rare zoonotic parasite that infects the liver of many mammals including humans. The aim of this study was to determine the seroprevalence of fascioliasis in Van province by ELISA (antibody detection) on the assumption that not all cases could be detected by stool examination alone. A total of randomly selected 1,600 patients, directed from affiliated outpatient clinics to Yüzüncü Yıl University Medical Faculty Parasitology Laboratory, were enrolled in the study. Their mean age was 44.44±19.00 years. Blood samples were collected from all the patients, and their stool samples were examined. For the stool examination, native-lugol and sedimentation (in formalin-ethyl acetate) methods were employed. ELISA for F. hepatica was performed on the blood samples from all patients. Seropositive patients were treated with triclabendazole. F. hepatica was detected by ELISA in 89 (5.6%) of the 1,600 patients, but eggs were identified on the stool examination in only 29 (1.8%) patients. The prevalence of F. hepatica was higher in females (7.2%) than in males (4.2%) and was higher in the ≥36-year age group (6.7%) than in the ≤35-year age group (4.4%). Abdominal pain (93.3%), fatigue (88.8%), and weight loss (69.7%) were the most common symptoms. Eosinophilia was present in 89.9% of the patients. All seropositive patients had a history of eating raw aquatic plants. Stool examination alone is not sufficient to diagnose F. hepatica. Serological tests such as ELISA must be used together with stool examination.

  6. Recurrence of cervical intraepithelial lesions after thermo-coagulation in HIV-positive and HIV-negative Nigerian women.

    The burden of cervical cancer remains huge globally, more so in sub-Saharan Africa. Effectiveness of screening, rates of recurrence following treatment and factors driving these in Africans have not been sufficiently studied. The purpose of this study therefore was to investigate factors associated with recurrence of cervical intraepithelial lesions following thermo-coagulation in HIV-positive and HIV-negative Nigerian women using Visual Inspection with Acetic Acid (VIA) or Lugol's Iodine (VILI) for diagnosis. A retrospective cohort study was conducted, recruiting participants from the cervical cancer "see and treat" program of IHVN. Data from 6 sites collected over a 4-year period was used. Inclusion criteria were: age ≥18 years, baseline HIV status known, VIA or VILI positive and thermo-coagulation done. Logistic regression was performed to examine the proportion of women with recurrence and to examine factors associated with recurrence. Out of 177 women included in study, 67.8 % (120/177) were HIV-positive and 32.2 % (57/177) were HIV-negative. Recurrence occurred in 16.4 % (29/177) of participants; this was 18.3 % (22/120) in HIV-positive women compared to 12.3 % (7/57) in HIV-negative women but this difference was not statistically significant (p-value 0.31). Women aged ≥30 years were much less likely to develop recurrence, adjusted OR = 0.34 (95 % CI = 0.13, 0.92). Among HIV-positive women, CD4 count thermo-coagulation occurs in a significant proportion of women. HIV-positive women with low CD4 counts are at increased risk of recurrent lesions and may be related to immunosuppression.

  7. Iodine-enhanced micro-CT imaging: methodological refinements for the study of the soft-tissue anatomy of post-embryonic vertebrates.

    The now widespread use of non-destructive X-ray computed tomography (CT) and micro-CT (µCT) has greatly augmented our ability to comprehensively detail and quantify the internal hard-tissue anatomy of vertebrates. However, the utility of X-ray imaging for gaining similar insights into vertebrate soft-tissue anatomy has yet to be fully realized due to the naturally low X-ray absorption of non-mineralized tissues. In this study, we show how a wide diversity of soft-tissue structures within the vertebrate head-including muscles, glands, fat deposits, perichondria, dural venous sinuses, white and gray matter of the brain, as well as cranial nerves and associated ganglia-can be rapidly visualized in their natural relationships with extraordinary levels of detail using iodine-enhanced (i-e) µCT imaging. To date, Lugol's iodine solution (I2 KI) has been used as a contrast agent for µCT imaging of small invertebrates, vertebrate embryos, and certain isolated parts of larger, post-embryonic vertebrates. These previous studies have all yielded promising results, but visualization of soft tissues in smaller invertebrate and embryonic vertebrate specimens has generally been more complete than that for larger, post-embryonic vertebrates. Our research builds on these previous studies by using high-energy µCT together with more highly concentrated I2 KI solutions and longer staining times to optimize the imaging and differentiation of soft tissues within the heads of post-embryonic archosaurs (Alligator mississippiensis and Dromaius novaehollandiae). We systematically quantify the intensities of tissue staining, demonstrate the range of anatomical structures that can be visualized, and generate a partial three-dimensional reconstruction of alligator cephalic soft-tissue anatomy. © 2014 Wiley Periodicals, Inc.

  8. Preoperative management in patients with Graves' disease.

    Graves' disease is the most frequent cause of hyperthyroidism in iodine-sufficient geographical areas and is characterized by the presence in patients' serum of autoantibodies directed against the thyrotropin receptor (TRAb) that cause overproduction and release of thyroid hormones. Clinical presentation results from both hyperthyroidism and underlying autoimmunity. The diagnosis is based on characteristic clinical features and biochemical abnormalities. If serum thyrotropin (TSH) is low, serum free thyroxine (FT4) and free triiodothyronine (FT3) concentrations should be measured to distinguish between subclinical (with normal circulating thyroid hormones) and overt hyperthyroidism (with increased circulating thyroid hormones). Graves' disease is treated with any of three effective and relatively safe initial treatment options: antithyroid drugs (ATDs), radioactive iodine ablation (RAIU), and surgery. Total thyroidectomy is favored in several clinical situations, such as intolerance, ineffectiveness or recurrence after ATD treatment, radioiodine therapy contraindicated, documented or suspected thyroid malignancy, one or more large thyroid nodules, coexisting moderate-to-severe active Graves' orbitopathy, women planning a pregnancy within 6 months. Whenever surgery is selected as treatment, selection of an expert high-volume thyroid surgeons is fundamental and careful preoperative management is essential to optimize surgical outcomes. Pretreatment with ATDs in order to promptly achieve the euthyroid state is recommended to avoid the risk of precipitating thyroid storm during surgery. For the majority of patients, euthyroidism is achieved after few weeks of ATD treatment. Beta-blockers, such as propranolol, are often added effectively to control hyperthyroid symptoms. Saturated solution of potassium iodide (SSKI) or potassium iodine (Lugol's solution), given for a short period prior to surgery, in order to reduce both thyroid hormone release and thyroid gland

    The radioactive contamination of Poznan region was recognized after Chernobyl accident as average. The predicted values of minimal (inhalation) and maximal (inhalation and ingestion) committed dose equivalent to the thyroid varied from 2.5 (min) to 24.7 (max) mSv in different groups of adults and children. To follow up the results of iodine prophylaxis and some aspects of possible thyroid gland morphological and functional changes 11086 persons were carefully investigated clinically and biochemically. Among these 11086 persons were 42.6% males and 57.4% females both adults from 17 till 40 year and children up to 16 years. The following parameters were reviewed: pregnancy, time of residence in the region, thyroid abnormalities, family history concerning thyroid diseases, iodine intake in April and May 1986 with possible side effects, changes in the thyroid size observed before and after 1986, degree and kind of thyroid enlargement, serum concentration of T 3 , T 4 , TSH, ATMA and ATG titration and finally the effectiveness of thyroid blockade at 24, 48 and 72 h after ingestion of Lugol's solution. Side effects of the ingestion of potassium iodine from 30-70 mg were observed in 153 cases, 36 of them consulted medicals doctors but in no case the side effects (dominated by vomiting) threatened the life. The data obtained indicated the existence in Poznan. Region the goiter endemic because 27.5% of investigated children and adults had goiter classified as grades O-B, I, II and III. The elevation or diminution of T 3 values were noted in 1164 cases, for T 4 in 418 cases and for TSH in 1412 cases.     Full Text Available Introducción: Se ha encontrado una prevalencia alta de parasitosis en escolares de la amazonía; pese a ello, no hay estudios realizados que revisen su asociación con el nivel socioeconómico y estado nutricional. Objetivos: Determinar la relación entre nivel de pobreza y estado nutricional con la presencia de parásitos intestinales en estudiantes escolares del caserío Venenillo, Huánuco. Diseño: Estudio transversal-analítico. Lugar: Caserío de Venenillo, Huánuco. Participantes: Estudiantes escolares de la única institución educativa del centro poblado. Intervenciones: A 42 estudiantes se les realizó examen coproparasitológico mediante observación directa con lugol y técnica de sedimentación rápida de Lumbreras. El nivel de pobreza fue determinado mediante el índice de necesidades básicas insatisfechas (NBI; y el grado de desnutrición, por medio del índice de Waterlow. El análisis estadístico fue realizado mediante el coeficiente gamma de Goodman y Kruskal. Principales medidas de resultados: Relación de parasitosis intestinal con pobreza y estado nutricional. Resultados: Los parásitos intestinales estuvieron presentes en 97,6%. Se encontró asociación significativa entre el nivel de parasitismo y el nivel de pobreza (p=0,02; gamma=0,82. No se encontró asociación significativa entre nivel de desnutrición y parasitismo (p=0,77. Conclusiones: La parasitosis intestinal se asoció al nivel de pobreza, no así con el nivel de desnutrición en la población de estudio.

    Full Text Available Objetivos: Determinar la prevalencia de infección por protozoarios y helmintos intestinales y su relación con el sexo, localidad de procedencia y grupo etáreo, en escolares nativos de la zona selvática del Alto Marañón, provincia de Bagua, departamento de Amazonas, Perú. Materiales y métodos: En este estudio transversal-observacional se seleccionó a los colegios representativos de la mencionada región lográndose examinar las muestras fecales de 1049 escolares de seis a 15 años de edad, entre octubre de 2001 y octubre de 2002, usando las técnicas microscópicas de observación directa, con solución salina fisiológica y lugol, de Teleman y de Kinnyoun. Resultados: La prevalencia de los protozoos y helmintos intestinales hallados es: Entamoeba coli 68,0%; Blastocystis hominis 28,4%; Endolimax nana 23,9%; Iodamoeba butschlii 32,9%; Giardia lamblia 21,4%; Entamoeba histolytica/dispar 12,9%; Cryptosporidium sp 01,9%; Paragonimus peruvianus 00,7% y Fasciola hepatica 00,2% (que no son parásitos intestinales sino de ubicación pulmonar y de vías biliares, respectivamente, pero sus huevos se encuentran en el contenido intestinal; Ancylostoma/Necartor 30,4%; Ascaris lumbricoides 28,9%; Trichocephalus trichiura 16,6%; Enterobius vermicularis 03,6%; Hymenolepis nana 03,5%; Taenia solium/saginata 00,2%; Strongyloides stercoralis 00,8% e H. diminuta 00,2%. Conclusiones: Ancylostoma/Necartor y A. lumbricoides se presentaron con mayor prevalencia en las poblaciones de menor edad de las comunidades de Mesones Muro y Nazaret, mientras que la infección humana por P. peruvianus se registra por primera vez en la región nororiental del Perú.

  12. Cross sectional study of serum selenium concentration and esophageal squamous dysplasia in western Kenya.

    Low serum selenium status has been associated with increased risk of esophageal squamous cell carcinoma (ESCC). East Africa is a region of high ESCC incidence and is known to have low soil selenium levels, but this association has not previously been evaluated. In this study we assessed the association of serum selenium concentration and the prevalence of esophageal squamous dysplasia (ESD), the precursor lesion of ESCC, in a cross-sectional study of subjects from Bomet, Kenya. 294 asymptomatic adult residents of Bomet, Kenya completed questionnaires and underwent endoscopy with Lugol's iodine staining and biopsy for detection of ESD. Serum selenium concentrations were measured by instrumental neutron activation analysis. Odds ratios (OR) and confidence intervals (95% CI) for associations between serum selenium and ESD were calculated using unconditional logistic regression. The mean serum selenium concentration was 85.5 (±28.3) μg/L. Forty-two ESD cases were identified (14% of those screened), including 5 (12%) in selenium quartile 1 (Q1), 5 (12%) in Q2, 15 (36%) in Q3, and 17 (40%) in Q4. Higher serum selenium was associated with prevalence of ESD (Q4 vs Q1: OR: 3.03; 95% CI: 1.05-8.74) and this association remained after adjusting for potential confounders (Q4 vs Q1: OR: 3.87; 95% CI: 1.06-14.19). This is the first study to evaluate the association of serum selenium concentration and esophageal squamous dysplasia in an African population at high risk for ESCC. We found a positive association between higher serum selenium concentration and prevalence of ESD, an association contrary to our original hypothesis. Further work is needed to better understand the role of selenium in the etiology of ESCC in this region, and to develop effective ESCC prevention and control strategies.

    Full Text Available Se cultivaron las microalgas dulceacuícolas Kirchneriella obesa (G. S. West Schmidle 1893, Scenedesmus quadricauda (Chodat, Turpin Bréb 1835 y Chlorococcum infusorium (Schrank Meneghini 1842, con el propósito comparar las tasas de crecimiento y su rendimiento en tres medios de cultivo, el F/2 de Guillard, Fert I y Fert II. El agua utilizada se pasó en filtros de cinco, uno y 0.45 mm, y se trató con luz ultravioleta. La temperatura varió de 24 a 25°C. Se cultivaron en un sistema de volumen creciente a partir de recipientes de 62.5 mL, duplicando el medio de cultivo cada 24 h, con el fin de completar 32 L en nueve días. Cada 24 h se obtuvieron tres muestras de 1 a 10 mL del cultivo, se preservaron en una solución de lugol y se calculó la concentración de las microalgas cada 24 h. El número total de células de las tres especies aumentó de manera exponencial hasta el final de los experimentos en los medios de cultivo F/2 y Fert I; esto no fue así con el medio Fert II, en el cual la tasa de duplicación y el número total de divisiones fueron más bajas. El medio F/2 y el Fert I ofrecieron mejores resultados que el Fert II. Los medios Fert I y Fert II tienen un valor económico más bajo que el medio F/2. El crecimiento y el rendimiento fueron mejores con el medio F/2, seguido por FI y FII.

  14. Genetic characterizations of Giardia duodenalis in sheep and goats in Heilongjiang Province, China and possibility of zoonotic transmission.

    Full Text Available BACKGROUND: Giardia duodenalis is a widespread intestinal protozoan of both humans and mammals. To date, few epidemiological studies have assessed the potential and importance of zoonotic transmission; and the human giardiasis burden attributable to G. duodenalis of animal origin is unclear. No information about occurrence and genotyping data of sheep and goat giardiasis is available in China. The aim of the present study was to determine prevalence and distribution of G. duodenalis in sheep and goats in Heilongjiang Province, China, and to characterize G. duodenalis isolates and assess the possibility of zoonotic transmission. METHODOLOGY/PRINCIPAL FINDINGS: A total of 678 fecal specimens were collected from sheep and goats on six farms ranging in age from one month to four years in Heilongjiang Province, China. The average prevalence of G. duodenalis infection was 5.0% (34/678 by microscopy after Lugol's iodine staining, with 5.6% (30/539 for the sheep versus 2.9% (4/139 for the goats. Molecular analysis was conducted on 34 G. duodenalis isolates based on the triosephosphate isomerase (tpi gene. 29 tpi gene sequences were successfully obtained and identified as assemblages A (n = 4, B (n = 2 and E (n = 23. High heterogeneity was observed within assemblage E at the tpi locus, with five novel subtypes found out of seven subtypes. Two subtypes of assemblage A were detected, including subtype AI (n = 3 and a novel subtype (designated as subtype AIV (n = 1. Two assemblage B isolates were identical to each other in the tpi gene sequences. CONCLUSIONS/SIGNIFICANCE: This is the first report of G. duodenalis infections in sheep and goats in China. The present data revealed the unique endemicity on prevalence, distribution and genetic characterization of G. duodenalis in sheep and goats in Heilongjiang Province. The findings of assemblages A and B in sheep and goats implied the potential of zoonotic transmission.

    Full text: An 123 I meta-iodobenzylguanidine (MIBG) scan was performed on a 54-year-old lady with familial phaeochromocytoma, to evaluate for bilateral or extra-adrenal disease. She has hypertension with raised catecholamines and CT evidence of a right adrenal phaeochromocytoma, and a female sibling with bilateral phaeochromocytoma. Thyroid blockade using Lugol's Iodine was given orally prior to intravenous administration of 370 MBq 123 I MIBG. Planar and SPECT imaging were acquired at 24 hours. There was intense uptake in the known right phaeochromocytoma. An unexpected finding was focal intense uptake in the region of the right thyroid lobe, which may be either a functioning paraganglioma arising from the cervical sympathetic ganglia or a medullary thyroid carcinoma (MTC). At 48 hours, a further image of the neck showed no changes. This was followed by a standard injection of 150 MBq 99 Tc m pertechnetate for thyroid scanning. Imaging obtained on 99 Tc m energy window setting showed a large hypofunctioning region in the right thyroid lobe, corresponding in location to the focal 123 I MIBG uptake. This is in keeping with a MTC, a neuroendocrine tumour, as other thyroid carcinomas are non-MIBG avid. A subsequent serum calcitonin assay showed elevated levels. The patient underwent surgical removal of the right phaeochromocytoma followed several weeks later by a right hemithyroidectomy. Histological reports confirmed the tumour diagnoses. The patient's familial phaeochromocytoma is therefore part of the multiple endocrine neoplasia syndrome Type 2A (MEN 2A). To date, biochemistry has not shown any evidence of hyperparathyroidism which occurs in 15-20 per cent of patients with MEN 2A.     This study investigated starch content, amount of pathogen DNA and density of occluded vessels in healthy and Verticillium dahliae infected olive shoots and stems. Starch hydrolysis is considered a mechanism to refill xylem vessels that suffered cavitation by either, drought conditions or pathogen infections. The main objective of this work was to evaluate this mechanism in olive plants subjected to V. dahliae infection or to drought conditions, in order to know the importance of cavitation in the development of wilting symptoms. In initial experiments starch content in the shoots was studied in trees of cultivars differing in the level of resistance growing in fields naturally infested with V. dahliae. The starch content, esteemed by microscopic observation of stem transversal sections stained with lugol, decreased with the level of symptom severity. Results were confirmed in a new experiment developed with young plants of cultivars ‘Picual’ (highly susceptible), ‘Arbequina’ (moderately susceptible) and ‘Frantoio’ (resistant), growing in pots under greenhouse conditions, either inoculated or not with V. dahliae. In this experiment, the pathogen DNA content, quantified by real-time PCR, and the density of occluded vessels, recorded by microscopic observations of transversal sections stained with toluidine blue, were related to the symptoms severity caused by the pathogen. Finally, a drought experiment was established with young plants of the cultivar ‘Picual’ grown in pots under greenhouse conditions in order to compare the effects caused by water deficit with those caused by the pathogen infection. In both cases, results show that starch hydrolysis occurred, what indirectly evidence the importance of xylem cavitation in the development of the symptoms caused by V. dahliae but in the water stressed plants no vessel occlusion was detected.   18. Evaluation of a cervical cancer screening program based on HPV testing and LLETZ excision in a low resource setting.

    Full Text Available We conducted studies in Vanuatu to evaluate potential screening and treatment strategies to assist with control of cervical cancer. In a pilot study of 496 women, visual inspection and cytology were evaluated as screening tests for detection of CIN 2 or worse (CIN2+, observed in 21 of 206 subjects biopsied on the basis of abnormal visual inspection or cytology. Sensitivity of visual inspection with Lugol's Iodine for detection of CIN2+ on biopsy was 0.63, specificity was 0.32, and the positive predictive value was 0.09. For HSIL cytology, sensitivity was 0.99, specificity was 0.77, and the positive predictive value was 0.88. HSIL cytology was significantly more sensitive and had a significantly higher PPV for CIN 2+ than visual inspection (p<0.01. In a further study of 514 women, we compared testing for HR HPV and cytology as predictors of biopsy proven CIN 2+. Sensitivity of HSIL cytology for CIN2+ as established by loop excision of the cervix was 0.81, specificity was 0.94, and positive predictive value was 0.48. Sensitivity of a positive test for HR HPV for detection of CIN2+ was non-significantly different from cytology at 0.81, specificity was 0.94, and positive predictive value was 0.42. Combining the two tests gave a significantly lower sensitivity of 0.63, a specificity of 0.98, and a positive predictive value of 0.68. For women over 30 in a low resource setting without access to cytology, a single locally conducted test for high risk HPV with effective intervention could reduce cervical cancer risk as effectively as intervention based on cytology conducted in an accredited laboratory.

    Blastocystosis is a frequent bowel disease. We planned to to evaluate the prevalence of Blastocystis spp. in patients who applied to the same internal medicine-gastroenterology clinic with or without gastrointestinal complaints to reveal the association of this parasite with diagnosed IBS and IBD. A total of 2334 patients with gastrointestinal symptoms composed the study group, which included 335 patients with diagnosed inflammatory bowel disease and 877 with irritable bowel syndrome. Patients without any gastrointestinal symptoms or disease (n = 192) composed the control group. Parasite presence was investigated by applying native-Lugol and formol ethyl acetate concentration to stool specimens, and trichrome staining method in suspicious cases. Blastocystis spp. was detected in 134 patients (5.74%) in the study group and 6 (3.12%) in the control group (p = 0.128). In the study group, Blastocystis spp. was detected at frequencies of 8.7% in ulcerative colitis (24/276), 6.78% in Crohn's disease (4/59), 5.82% in irritable bowel syndrome (51/877), and 4.9% in the remaining patients with gastrointestinal symptoms (55/1122). Blastocystis spp. was detected at a statistically significant ratio in the inflammatory bowel disease (odds ratio [OR] = 2.824; 95% confidence interval [CI]: 1.149-6.944; p = 0.019) and ulcerative colitis (OR = 2.952; 95% CI: 1.183-7.367; p = 0.016) patients within this group compared to controls. There were no statistically significant differences between the control group and Crohn's disease or irritable bowel syndrome patients in terms Blastocystis spp. frequency (p = 0.251, p = 0.133). Blastocystosis was more frequent in patients with inflammatory bowel disease, especially those with ulcerative colitis. Although symptomatic irritable bowel syndrome and Crohn's disease patients had higher rates of Blastocystis spp. infection, the differences were not significant when compared to controls.

    The dysfunction of myocardial sympathetic system was implied in occurrence of ventricular arrhythmias in patients with a right ventricle arrhythmia-gen dysplasia (RVAD). The goal of this study is to evaluate the myocardial pre-synaptic adrenergic regional function by using the cardiac tomo-scintigraphy with {sup 123}I - meta-iodo-benzyl-guanidine (MIBG). Fourteen patients (12 M, 2 F, age: 46{+-} 13) in whom the disease`s diagnosis was done on the basis of the parameters of European Task Force (electric, angiographic, histologic), were studied. Six healthy subjects (32 {+-} 12 years) were at the same time studied as control group. Each patient benefited by an at-rest tomo-scintigraphy by {sup 201}Tl to eliminate any hypo-perfusion which could hinder the interpretation of MIBG fixation. A 48 h delay was necessary between the two isotopic examinations. After blocking the thyroid by Lugol fort, an at-rest tomo-scintigraphy by {sup 123}I - MIBG was effected 4 h after the injection IV of 259 MBq of tracer following a classical acquisition of a myocardial tomography with a tracer-adopted collimator (Elscint camera). Global and regional evaluations of the cardiac adrenergic neuronal function were effected by using the cardio-mediastinal ratio (CMR) and the circumferential profile, respectively, for the localization, extension and amplitude of regional adrenergic defects. The CMR is within the normal limits (236 {+-} 39% vs 234 {+-} 14% in healthy subjects). The adrenergic defects are present in 11/14 patients (a reduction of 50% of capture of MIBG as compared with the control group, < average - 2 ET) in the anterior and lateral regions of left ventricle. In conclusion, the tomo-scintigraphy by {sup 123}I - MIBG allows the detecting of presence of a sympathetic dys-innervation of left ventricle in patients afflicted with RVAD

    Full Text Available En la actualidad, existe un incremento del consumo de comida fuera del núcleo familiar. Sin embargo, los vegetales que se consumen crudos en los establecimientos de “comida rápida” son un vehículo potencial para adquirir parásitos intestinales de interés médico-zoonótico. Entre enero y mayo de 2009, se evaluaron parasitológicamente mediante lavado con agua destilada estéril + Tween® 20 al 20 %, sedimentación espontánea por 24 horas, centrifugación y la observación microscópica de tinciones de lugol y Kinyoun, 45 muestras de vegetales correspondientes a 3 especies (tomate, lechuga, repollo y una mezcla (repollo + zanahoria de hortalizas, las cuales se obtuvieron al azar en 34 establecimientos de “comida rápida” de la ciudad de Coro, Estado Falcón, Venezuela. Se detectó una prevalencia global del 77,78 % (35/45, siendo el repollo (100 % y la lechuga (80 % las hortalizas que presentaron mayores porcentajes de contaminación parasitaria. Los parásitos intestinales más frecuentemente observados fueron: Entamoeba histolytica/dispar/moshkovskii (22,22 %, Blastocystis sp. (22,22 %, Giardia sp. (20,00 % y los Ancylostomídeos (13,33 %. No se encontró una relación estadísticamente significativa y directa entre los porcentajes de contaminación y las variables epidemiológicas evaluadas (p > 0,05, sugiriendo que otros factores, tanto abióticos como bióticos, incluyendo los sinantrópicos, podrían estar influenciando en la contaminación de las hortalizas. Se recomienda implementar un plan sanitario más estricto en los establecimientos de “comida rápida”, y programas de educación para la salud de la población a mayor riesgo de adquirir las parasitosis intestinales.

Sin embargo, los vegetales que se consumen crudos en los establecimientos de “comida rápida” son un vehículo potencial para adquirir parásitos intestinales de interés médico-zoonótico. Entre enero y mayo de 2009, se evaluaron parasitológicamente mediante lavado con agua destilada estéril + Tween® 20 al 20 %, sedimentación espontánea por 24 horas, centrifugación y la observación microscópica de tinciones de lugol y Kinyoun, 45 muestras de vegetales correspondientes a 3 especies (tomate, lechuga, repollo y una mezcla (repollo + zanahoria de hortalizas, las cuales se obtuvieron al azar en 34 establecimientos de “comida rápida” de la ciudad de Coro, Estado Falcón, Venezuela. Se detectó una prevalencia global del 77,78 % (35/45, siendo el repollo (100 % y la lechuga (80 % las hortalizas que presentaron mayores porcentajes de contaminación parasitaria. Los parásitos intestinales más frecuentemente observados fueron: Entamoeba histolytica/dispar/moshkovskii (22,22 %, Blastocystis sp. (22,22 %, Giardia sp. (20,00 % y los Ancylostomídeos (13,33 %. No se encontró una relación estadísticamente significativa y directa entre los porcentajes de contaminación y las variables epidemiológicas evaluadas (p > 0,05, sugiriendo que otros factores, tanto abióticos como bióticos, incluyendo los sinantrópicos, podrían estar influenciando en la contaminación de las hortalizas. Se recomienda implementar un plan sanitario más estricto en los establecimientos de “comida rápida”, y programas de educación para la salud de la población a mayor riesgo de adquirir las parasitosis intestinales.

  3. The Spatial Predilection for Early Esophageal Squamous Cell Neoplasia: A "Hot Zone" for Endoscopic Screening and Surveillance.

    Early esophageal squamous cell neoplasias (ESCNs) are easily missed with conventional white-light endoscopy. This study aimed to assess whether early ESCNs have a spatial predilection and the patterns of recurrence after endoscopic treatment. We analyzed the circumferential and longitudinal location of early ESCNs, as well as their correlations with exposure to carcinogens in a cohort of 162 subjects with 248 early ESCNs; 219 of which were identified by screening and 29 by surveillance endoscopy. The circumferential location was identified using a clock-face orientation, and the longitudinal location was identified according to the distance from the incisor. The most common circumferential and longitudinal distributions of the early ESCNs were found in the 6 to 9 o'clock quadrant (38.5%) and at 26 to 30 cm from the incisor (41.3%), respectively. A total of 163 lesions (75%) were located in the lower hemisphere arc, and 149 (68.4%) were located at 26 to 35 cm from the incisor. One hundred eleven (51%) early ESCNs were centered within the "hot zone" (i.e., lower hemisphere arc of the esophagus at 26 to 35 cm from the incisor), which comprised 20% of the esophageal area. Exposure to alcohol, betel nut, or cigarette was risk factors for the development of early ESCNs in the lower hemisphere. After complete endoscopic treatment, the mean annual incidence of metachronous tumors was 10%. In addition, 43% of the metachronous recurrent neoplasias developed within the "hot zone." Cox regression analysis revealed that the index tumor within the hot zone (hazard ratio [HR]: 3.19; 95% confidence interval [CI]: 1.17-8.68; P = 0.02) and the presence of numerous Lugol-voiding lesions in the esophageal background mucosa were independent predictors for metachronous recurrence (HR: 4.61; 95% CI: 1.36-15.56; P = 0.01).   4. Blastocystosis in patients with gastrointestinal symptoms: a case–control study

    Background Blastocystosis is a frequent bowel disease. We planned to to evaluate the prevalence of Blastocystis spp. in patients who applied to the same internal medicine-gastroenterology clinic with or without gastrointestinal complaints to reveal the association of this parasite with diagnosed IBS and IBD. Methods A total of 2334 patients with gastrointestinal symptoms composed the study group, which included 335 patients with diagnosed inflammatory bowel disease and 877 with irritable bowel syndrome. Patients without any gastrointestinal symptoms or disease (n = 192) composed the control group. Parasite presence was investigated by applying native-Lugol and formol ethyl acetate concentration to stool specimens, and trichrome staining method in suspicious cases. Results Blastocystis spp. was detected in 134 patients (5.74%) in the study group and 6 (3.12%) in the control group (p = 0.128). In the study group, Blastocystis spp. was detected at frequencies of 8.7% in ulcerative colitis (24/276), 6.78% in Crohn’s disease (4/59), 5.82% in irritable bowel syndrome (51/877), and 4.9% in the remaining patients with gastrointestinal symptoms (55/1122). Blastocystis spp. was detected at a statistically significant ratio in the inflammatory bowel disease (odds ratio [OR] = 2.824; 95% confidence interval [CI]: 1.149-6.944; p = 0.019) and ulcerative colitis (OR = 2.952; 95% CI: 1.183-7.367; p = 0.016) patients within this group compared to controls. There were no statistically significant differences between the control group and Crohn’s disease or irritable bowel syndrome patients in terms Blastocystis spp. frequency (p = 0.251, p = 0.133). Conclusions Blastocystosis was more frequent in patients with inflammatory bowel disease, especially those with ulcerative colitis.     Full text: Objective: The poster highlights the importance of scintigraphy with 123 I-MIBG in the complete evaluation of a patient with Pepper syndrome. Pepper syndrome is a particular form of the metastatic neuroblastoma in stage 4S (hepatic metastasis without bone involvement) with a good prognosis, which affects mainly newborns. MIBG (metaiodobenzyl-guanidine) being an analogue of norephinephrine - is captured (active mechanism) by the neuroendocrine cells and is concentrated in the medula of the adrenal glands, sympathetic paraganglia, myocardium sympathetic nervous terminations, APUD system and in the tumours originating from these structures. Materials and Methods: EN, 3 months old, feminine sex, suspected of having 4S stage neuroblastoma (Pepper syndrome) has been evaluated by a scintigraphic examination 24 hours after intravenous injection of 80 MBq 123 I-MIBG. We used a dual-head GE gamma camera, LEUHR collimator with parallels channels for low energy, with high resolution; energy window:159 keV +/- 10%. We performed static images of the cranium (anterior, posterior and lateral view), thorax, abdomen, pelvis and limbs (anterior and posterior view). Thyroid gland was blocked with Lugol. We have repeated the scintigraphic examinations at 3, 4, 6 and 7 months after the initial exam. Results and Discussions: First examination confirmed the diagnosis of Pepper syndrome putting in evidence a hyper fixation of the radio pharmaceutic in the right abdominal side corresponding with the primitive tumour of the adrenal medula; a heterogeneous hyper fixation of the liver evoking a multi nodular secondary hepatomegaly and some hyper fixations focalized to the cranial box (frontal right), to the superior epiphysis of the left tibia and to the superior extremity of the left femur (osteomedullary invasion).     Full Text Available Phytoplankton communities of three water bodies in the Lednice park were studied from 22nd April till 1st October 2002. These water bodies are the Zámecký pond, Růžový pond and the Dyje River, which is water source of both ponds.Phytoplankton samples were taken every two weeks between 8 - 9 am. Collected phytoplankton samples were preserved with 4% formalin solution and Lugol solution (JJK and transported to the laboratory. They were determined and counted using inverted microscope. Water temperature, pH and dissolved oxygen were measured in the field using digital portable instruments. Total of 317 phytoplankton species were determined in this study.Heavy algal bloom was observed in the Zámecký pond in mid-summer coinciding with increase in water temperature. Fish diseases and partial mortality occurred during the period of algal bloom and unpleasant smell was dominant feature. A light algal bloom was also observed in the Růžový pond and the Dyje River nearly by the end of summer.The main algae species responsible for blue-green algae bloom were Anabaena flos-aquae, Microcystis aeruginosa, M. ichtyoblabe, M. flos-aquae and M. wesenbergii. Dissolved oxygen values varied between 3.4 - 19.5 mg l-1, pH ranged from 7.6 - 9.7. Secchi depth varied from 0- 65 cm in the Zámecký pond, 15-45 cm in the Růžový pond and 35-65 cm in the Dyje River. Concentration of total phosphate, nitrate and chlorophyll-a in the Dyje River before drainage into the Zámecký and Růžový pond verified heavy nutrient load (Total-P = 0.3, NO3- = 12 mg.l-1 of the river. Although the Dyje River is main water source for both ponds, presence of relatively different phytoplankton communities in these two ponds suggest that probably different nutrient sources might be responsible for differences in phytoplankton communities and eutrophication patterns in the Zámecký pond as compared to the Růžový pond.

    Full Text Available The present study involved the measurement of size and the micromorphology of the floral elements of Anchusa officinalis L. which are attractants for insects. The structure of the epidermis on the surface of the calyx, petals, throat scales, pistil and nectary were analysed using light and scanning electron microscopy (SEM. For light microscopy observations, semi-permanent slides were prepared, which were treated with Lugol's iodine solution, Sudan III and fluoroglucine. The dark violet lobes of the corolla of Anchusa officinalis, with a velvety surface, and the throat scales, contrasting with them, belong to the most important optical attractants which lure insects from large distances. The dark pink colouring of the sepals additionally increases the attractiveness of the flowers. The epidermis covering the calyx formed different-sized non-glandular trichomes as well as glandular trichomes. The glandular trichomes were composed of a uni - or bicellular leg and a unicellular head. The colour of the corolla petals was determined by anthocyanins accumulated in the epidermal cells and in the more deeply situated parenchyma. The velvety surface was formed by the conical papillae, densely growing from the adaxial epidermis. The pink-violet throat scales with white hairs, covering the inlet to the tube of the corolla, were found at the inlet to the corolla throat. The longest trichomes on the surface of the scales were located in their lower and middle parts, whereas the shortest ones at their tips. The epidermis of the central part of the throat scales formed small papillae. The trichomes had thin cell walls, large vacuoles, numerous plastids and lipid droplets. The two-parted stigma of the pistil was covered by characteristic expanded outgrowths with wavy edges which performed the functions of structures facilitating the capture of pollen grains.     The World Health Organization (WHO)'s cervical cancer screening guidelines for limited-resource settings recommend sequential screening followed by same-day treatment under a "screen-and-treat" approach. We aimed to (1) assess feasibility and clinical outcomes of screening HIV-positive and HIV-negative Cameroonian women by pairing visual inspection with acetic acid and Lugol's iodine enhanced by digital cervicography (VIA/VILI-DC) with careHPV, a high-risk human papillomavirus (HR-HPV) nucleic acid test designed for low-resource settings; and (2) determine persistence of HR-HPV infection after one-year follow-up to inform optimal screening, treatment, and follow-up algorithms. We co-tested 913 previously unscreened women aged ≥30years and applied WHO-recommended treatment for all VIA/VILI-DC-positive women. Baseline prevalence of HR-HPV and HIV were 24% and 42%, respectively. On initial screen, 44 (5%) women were VIA/VILI-DC-positive, of whom 22 had HR-HPV infection, indicating 50% of women screened false-positive and would have been triaged for unnecessary same-day treatment. VIA/VILI-DC-positive women with HIV infection were three times more likely to be HR-HPV-positive than HIV-negative women (65% vs. 20%). All women positive for either VIA/VILI-DC or HR-HPV (n=245) were invited for repeat co-testing after one year, of which 136 (56%) returned for follow-up. Of 122 women who were HR-HPV-positive on initial screen, 60 (49%) re-tested negative, of whom 6 had received treatment after initial screen, indicating that 44% of initially HR-HPV-positive women spontaneously cleared infection after one year without treatment. Women with HIV were more likely to remain HR-HPV-positive on follow-up than HIV-negative women (61% vs. 22%, p<0.001). Treatment was offered to all VIA/VILI-DC positive women on initial screen, and to all women screening VIA/VILI-DC or HR-HPV positive on follow-up.     Screening is the main preventive method for cervical cancer in developing countries, but each type of screening has advantages and disadvantages. To investigate the most suitable method for low-income areas in China, we conducted a health economic analysis comparing three methods: visual inspection with acetic acid and Lugol's iodine (VIA/VILI), ThinPrep cytology test (TCT), and human papillomavirus (HPV) test. We recruited 3086 women aged 35-65 years using cluster random sampling. Each participant was randomly assigned to one of three cervical cancer screening groups: VIA/VILI, TCT, or HPV test. In order to calculate the number of disability-adjusted life years (DALYs) averted by each screening method, we used Markov models to estimate the natural development of cervical cancer over a 15-year period to estimate the age of onset and duration of each disease stage. The cost-effectiveness ratios (CERs), net present values (NPVs), benefit-cost ratios (BCRs), and cost-utility ratios (CURs) were used as outcomes in the health economic analysis. The positive detection rate in the VIA/VILI group was 1.39%, which was 4.6 and 2.0 times higher than the rates in the TCT and HPV test groups, respectively. The positive predictive value of VIA/VILI (10.53%) was highest while the rate of referral for colposcopy was lowest for those in the HPV + TCT group (0.60%). VIA/VILI performed the best in terms of health economic evaluation results, as the cost of per positive case detected was 8467.9 RMB, which was 24503.0 RMB lower than that for TCT and 5755.9 RMB lower than that for the HPV test. In addition, the NPV and BCR values were 258011.5 RMB and 3.18 (the highest), and the CUR was 2341.8 RMB (the lowest). The TCT performed the worst, since its NPV was <0 and the BCR was <1, indicative of being poorly cost-beneficial. With the best economic evaluation results and requiring minimum medical resources, VIA/VILI is recommended for cervical cancer screening in poverty-stricken areas

    Cervical cancer is the leading cause of cancer morbidity and mortality in women in sub-Saharan Africa (SSA), accounting for about 50,000 deaths annually. Until recently, cytology was the gold standard for screening and prevention of cervical cancer. This method of screening has not been successful in SSA due to a lack of human, financial and material resources and poor health care infrastructure. It is estimated that less than 5% of at risk women have ever being screened. In the past two decades alternative approaches to cytology for cervical cancer screening have been evaluated in low- and medium-income countries. Visual inspection with acetic acid (VIA) and/or Lugol's iodine (VILI) have been shown to have adequate sensitivity, although low specificity, in a number of cross-sectional research and demonstration projects. Visual inspection methods require minimal resources, are technologically accessible, and are feasible for screening for precancerous lesions. Linking screening with VIA/VILI to treatment with cryotherapy may enable screening and treatment to take place in one visit, but this is likely to result in large numbers of women being subjected to unnecessary treatment. A number of studies have shown that cryotherapy is not associated with significant side effects or complications and is well tolerated. Creating the infrastructure for screening of older women is considered desirable, despite the limitations of visual inspection methods as screening tests. Understanding the role of human papillomavirus (HPV) infection in the etiology of cervical cancer and the discovery of HPV rapid test kits, as well as the development of vaccines against the HPV oncogenic types, have created new opportunities for prevention of cervical cancer. Trials and projects have established (and are still ongoing) the feasibility of using these molecular tests for screening.   12. Early detection of cervical cancer with visual inspection methods: a summary of completed and on-going studies in India

    Full Text Available India is a high-risk country for cervical cancer which accounts a quarter (126 000 new cases, 71 000 deaths around 2 000 of the world burden. The age-standardized incidence rates range from 16-55 per 100 000 women in different regions with particularly high rates in rural areas. Control of cervical cancer by early detection and treatment is a priority of the National Cancer Control Programme of India. There are no organized cytology screening programmes in the country. The technical and financial constraints to organize cytology screening have encouraged the evaluation of visual inspection approaches as potential alternatives to cervical cytology in India. Four types of visual detection approaches for cervical neoplasia are investigated in India: a naked eye inspection without acetic acid application, widely known as 'downstaging'; b naked eye inspection after application of 3-5% acetic acid (VIA; c VIA using magnification devices (VIAM; d visual inspection after the application of Lugol's iodine (VILI. Downstaging has been shown to be poorly sensitive and specific to detect cervical neoplasia and is no longer considered as a suitable screening test for cervical cancer. VIA, VIAM and VILI are currently being investigated in multicentre cross-sectional studies (without verification bias, in which cytology and HPV testing are also simultaneously evaluated, and the results of these investigations will be available in 2003. These studies will provide valuable information on the average, comparative test performances in detecting high-grade cervical cancer precursors and cancer. Results from pooled analysis of data from two completed studies indicated an approximate sensitivity of 93.4% and specificity of 85.1% for VIA to detect CIN 2 or worse lesions; the corresponding figures for cytology were 72.1% and 91.6%.     Full Text Available The aim of this work was to study the phytoplankton community composition at limnetic environment in Pernambuco, Brazil. Samplings were carried out from April/2001 to March/2002. Samples to analyses the biotic variables were taken using a recipient with a large overture, at the subsurface and with a Van Dorn bottle at the bottom. The rainfall data were recorded and the water transparency was used to calculate the light attenuation coefficient, photic zone and the determination of trophic state index. The concentrations of total phosphorus and total nitrogen were determined in a typical dry and rainy month. Equitability, also the, similarity and diversity indexes and the densities and correlation of total densities among depths were calculated. Forty-five taxa were identified in Chlorophyta (21spp, Cyanophyta (17spp and Bacillariophyta (7spp, while flagellates were quantified in groups without identification. Cyanophyta presented highest diversity at both the depths and Planktothrix agardhii was the highest density species. Significant differences were not observed between subsurface and bottom densities. Results showed that the reservoir was eutrophicated and presented high densities of Cyanophyta.O objetivo deste trabalho foi o estudo da comunidade fitoplanctônica em ambiente limnético de Pernambuco, Brasil. O presente trabalho teve por objetivo estudar os efeitos da aplicação de ácido indol butírico (AIB, em solução e em pó, no enraizamento de estacas de espinheira-santa coletadas nas quatro estações do ano (abril/2005 a janeiro/2006, bem como averiguar, por meio de análises anatômicas e histoquímicas das estacas, a presença de possíveis impedimentos à iniciação do enraizamento adventício. Estacas provenientes de ramos de plantas matrizes de seis anos cultivadas da Estação Experimental do Canguiri, Pinhais, PR, foram coletadas e tratadas com AIB (0, 1500, 3000 mg L-1 ou mg kg-1, em solução alcoólica (50% v/v e em talco. Aos 365 dias foram avaliadas as porcentagens de estacas enraizadas e mortas, número e comprimento médio de raízes formadas por estaca. Análises anatômicas e histoquímicas com lugol e cloreto férrico foram realizadas. A estação mais promissora para o enraizamento foi o verão/2006 com 62,50% para o tratamento controle, devido à menor lignficação dos ramos no período de intenso crescimento vegetativo. O número médio de raízes formadas por estaca foi de 6,94 (solução e o comprimento médio de raízes formadas/estaca chegou a 4,82 cm nesta mesma estação.     Full Text Available Introducción. No se han descrito las fuentes potenciales de infección por Blastocystis sp. en Colombia. Objetivos. Estimar la frecuencia de Blastocystis sp. y explorar posibles asociaciones entre la infección, las manifestaciones clínicas y algunas fuentes ambientales. Materiales y métodos. Se llevó a cabo un estudio descriptivo con una población de 1.993 menores de 5 años (lactantes y preescolares de estratos socioeconómicos 1 y 2, atendidos en hogares infantiles de Calarcá, Colombia. Se calculó una muestra de 275 y se seleccionó aleatoriamente a los participantes en 10 % de los hogares infantiles. Se practicaron exámenes coprológicos y encuestas y se tomaron muestras para la identificación del parásito en heces, uñas, animales domésticos y algunas fuentes ambientales. Se hicieron preparaciones en fresco con lugol al 1 % y se utilizó una técnica de concentración con formol-éter. Se estimaron prevalencias y razones de momios; para las variables cualitativas se utilizó el test de ji al cuadrado y, para las cuantitativas, la prueba t de Student. Resultados. La prevalencia de infección en los niños fue de 57,5 %. Hubo asociación estadísticamente significativa entre la presencia del parásito en heces y su hallazgo en recipientes de agua de panela (OR=1,70; IC95% 1,05-2,79, en los pisos de madera o de tierra en la vivienda (OR=3,43; IC95% 1,12-10,48, y en alimentos diferentes a la leche en el tetero (OR=3,23; IC95% 1,41-4,41; y se encontró una prevalencia alta sin asociación en muestras de uñas (OR=1,62; IC95% 0,99-2,67. La diarrea fue más frecuente en niños con Blastocystis sp. (45 % que en aquellos sin el parásito (33 % (OR: 1,95; IC95% 0,99-2,77 (p<0,05. Conclusiones.     Full Text Available India is a high-risk country for cervical cancer which accounts a quarter (126 000 new cases, 71 000 deaths around 2 000 of the world burden. The age-standardized incidence rates range from 16-55 per 100 000 women in different regions with particularly high rates in rural areas. Control of cervical cancer by early detection and treatment is a priority of the National Cancer Control Programme of India. There are no organized cytology screening programmes in the country. The technical and financial constraints to organize cytology screening have encouraged the evaluation of visual inspection approaches as potential alternatives to cervical cytology in India. Four types of visual detection approaches for cervical neoplasia are investigated in India: a naked eye inspection without acetic acid application, widely known as 'downstaging'; b naked eye inspection after application of 3-5% acetic acid (VIA; c VIA using magnification devices (VIAM; d visual inspection after the application of Lugol's iodine (VILI. Downstaging has been shown to be poorly sensitive and specific to detect cervical neoplasia and is no longer considered as a suitable screening test for cervical cancer. VIA, VIAM and VILI are currently being investigated in multicentre cross-sectional studies (without verification bias, in which cytology and HPV testing are also simultaneously evaluated, and the results of these investigations will be available in 2003. These studies will provide valuable information on the average, comparative test performances in detecting high-grade cervical cancer precursors and cancer. Results from pooled analysis of data from two completed studies indicated an approximate sensitivity of 93.4% and specificity of 85.1% for VIA to detect CIN 2 or worse lesions; the corresponding figures for cytology were 72.1% and 91.6%.   18. A familial outbreak of fascioliasis in Eastern Anatolia: a report with review of literature.

    To present the results of a cross-sectional epidemiological analysis of a familial outbreak of fascioliasis in Eastern Anatolia and to discuss the clinical, diagnostic and therapeutic properties of the patients. A screening group consisting of 92 individuals from the same family with a history of watercress ingestion and a control group consisting of 30 individuals from neighboring families were included in the study. In both groups, full blood count, erythrocyte sedimentation rate, liver function tests and total IgE levels were assessed. Stool analysis was performed on three consecutive days with native, lugol and sedimentation methods. The diagnosis was based on the detection of parasite ova in the stool or alternatively based on consistent clinical, laboratory, and radiological findings or positive clinical findings in combination with a positive ELISA test. Abdominal ultrasonography and computerized tomography scans were performed on all patients. 24 patients (21 women and 3 men) were diagnosed with fascioliasis. The mean age was 24.5±18.6 years (range, 5-64 years). All cases had a history of watercress ingestion, malaise, fatigue, lack of appetite, and abdominal pain. Clinical features included: weight loss was present in 18 cases (75%), dyspepsia in 12 (50%), headache in 11 (45.8%), sweating in 10 (41.7%), fever and dyspnea each in 8 (33.3%), nausea and vomiting in 6 (25%), and itching in 4 (16.75). The most common laboratory abnormalities were total IgE elevation in 19 cases (79.2%) and eosinophilia in 17 (70.8%). The eosinophilia was >20% in 14 cases (58.3%) and the total IgE was >500IU/ml in 15 cases (62.5%). Stool examination for ova was positive in 11 cases. 10 patients had positive clinical, laboratory and radiological findings. A further three patients were diagnosed based on their clinical findings and their ELISA results. All cases had positive ELISA results.   19. Effect of low dose 131I-MIBG therapy in metastatic neural crest tumors: Evaluation by RECIST and quality of life questionnaire

    Full text: The primary aim of 131I-MIBG therapy in advanced metastatic or recurrent neural crest tumors is palliation i.e. disease control and improvement of health related quality of life. No clear guidelines regarding the dosage and schedule of 131I-MIBG therapy in neural crest tumors exist at present. In general, a fixed dose of 100-300 mCi has been suggested for each therapy. We share our experience of 131I-MIBG therapy in various subgroups of neural crest tumors and discuss the response assessed by the RECIST and the quality of life questionnaire. A total number of 14 patients were treated with indigenously produced 131I-MIBG, which was administered as continuous intravenous infusion over a period of 2-4 hours. Patient isolation according to guidelines set by the national regulatory authority and thyroid blockade with Lugol's iodide were strictly adhered to. Antihypertensive measures were undertaken in case of pheochromocytoma and paraganglioma to prevent effects of catecholamine release during or following 131I-MIBG infusion. The primaries included neuroblastoma (n=7), pheochromocytoma (n=5) and paraganglioma (n=2). The cases of neuroblastoma included patients with progressive disease where the conventional chemotherapy had failed, while those of pheochromocytoma and paraganglioma were cases with recurrent / metastatic disease following surgery. In cases of multiple therapies, the minimum interval between two consecutive therapies was 12 weeks. Regular renal and haematological profiles were monitored in all the cases. Response to therapy was assessed by RECIST. The findings of 131I-MIBG scintigraphy were incorporated with CT scan in assessing the target lesions. Biochemical response was evaluated by 24 hours urinary VMA estimation. The quality of life status was evaluated by the conventional questionnaire. A total of 27 therapies were administered in 14 patients. In five treated cases of pheochromocytoma, three received multiple therapies. Follow up results

    Full Text Available Los estudios sobre aspectos reproductivos no son muy abundantes en la literatura científica sobre los taxones de Lycopodiaceae y constituyen información esencial para apoyar la taxonomía y relaciones sistemáticas en el grupo. Por lo tanto, se presenta aquí un análisis detallado de la ontogenia de los esporangios y esporogénesis, así como determinaciones químicas de varios compuestos generados durante la formación de las esporas. Los análisis se llevaron a cabo en 14 taxones de seis géneros de la familia: Diphasiastrum, Diphasium, Huperzia (un género que se trata aquí, incluyendo Phlegmariurus, Lycopodiella, Lycopodium y Palhinhaea. Las muestras fueron recolectadas en tres departamentos de los Andes de Colombia entre 1 454-3 677m de altitud. La ontogenia se estudió en trozos de estróbilos y ejes, de 1cm de largo, que se fijaron en glutaraldehido o FAA, se deshidrataron en alcohol, se incluyeron en LR White, se seccionaron en cortes de 0.2-0.5μm y se colorearon con azul de toluidina (TBO, un colorante metacromático que permite detectar tanto esporopolenina como lignina o sus precursores. Para estudios adicionales, secciones de 3-5μm de material incluido en paraplast plus se colorearon con safranina-verde rápido y azul alciánhematoxilina.     [目的]通过分析襄垣县2009~2015年农村妇女宫颈癌筛查结果,为国家宫颈癌筛查项目的评价和优化提供理论依据.[方法]对襄垣县35~64岁妇女开展宫颈癌筛查.2009~2013年采用醋酸/碘染色后肉眼观察(VIA/VILI),2014年开始部分引入人乳头状瘤病毒(HPV)检测,2015年又引入新柏氏液基细胞学(TCT)检测,根据卫生资源情况在不同人群中分别使用三种方法.任意筛查结果阳性者转诊阴道镜,镜下有病变时取活检,以病理诊断为金标准.通过评价不同方法对宫颈上皮内瘤变(CIN)2级及以上病变的检出率评价不同方法对宫颈癌的筛查效果.[结果] 2009~2015年共筛查62 618名女性,其中CIN2级及以上(CIN2+)病变的患病率为0.76%,早诊率为90.53%,下生殖道感染率为25.68%,其中滴虫性阴道炎、细菌性阴道炎和宫颈炎与HPV感染相关.根据2014及2015年数据,VIA/VILI的CIN2+病变检出率为0.74%,TCT的病变检出率为0.70%,两者相似,但均低于HPV检测,其病变检出率为1.37%.[结论]HPV检测是首选的宫颈癌初筛方法,在资源匮乏地区,培训合格的基层医生使用VIA/VILI或TCT方法对适龄妇女开展宫颈癌筛查是有效的备选方案.%[Purpose] To analyze the results of Cervical Cancer Screening Program for Rural Women in Xiangyuan county,Shanxi province during 2009 to 2015.[Methods] According to the protocol of the National Cervical Cancer Screening Program in Rural China,women aged 35 to 64 in Xiangyuan county were recruited for screening.Visual inspection with acetic acid and Lugol's iodine(VIA/VILI) was used in 2009 to 2013,human papillomavirus(HPV) DNA test was introduced in 2014 and then thin-Prep cytology(TCT) was introduced in 2015.Colposcopic examination was performed if the result was positive.Cervical lesions were diagnosed by direct biopsy under the colposcopy,and the pathological diagnosis was used for gold standard.The screening effectiveness was

    Full Text Available As an ardent cancer researcher, Dr. Smita Asthana has a vision to create wider awareness on cancer and its prevention, and aims to work on translational research to benefit the general public through the implementation of evidence-based research. “I have been associated with the National Institute of Cancer Prevention and Research (NICPR and Institute of Cytology and Preventive Oncology (ICPO since November 2004 and have progressed over a period of time from being a staff scientist to the current role of a senior scientist,” says Dr. Asthana, who is presently with NICPR’s Biostatistics and Epidemiology division.“I have been working in various positions that deal with the design, execution, and evaluation of medical projects. Recently, we have concluded two major cervical cancer screening projects and conducted a screening of 10,000 women in rural areas,” she tells AMOR.     the presence of Giardia cysts; 447 faecal samples were taken from 232 persons coming from 5 cities situated in Wielkopolska, and 1736 faecal samples were obtained from 123 animal species, including: 266 faecal samples from 113 species of animals kept in the Zoological Garden in Poznań, 1286 samples from 4 species of breeding animals, 118 samples from dogs, and 66 samples from 5 species of wild animals. Faecal samples were taken from animals coming from 25 places in Wielkopolska. Moreover, seven isolates of G. intestinalis were used in the studies, which were obtained from various species of hosts and kept in an axenic in vitro culture. Microscopic, molecular and bio-informative methods were used in the studies. From each faecal sample fresh smears were made in a 0.6% solution of physiological salt and in Lugol's solution, as well as a permanent smear stained with trichrome was made. Moreover, the following molecular techniques were implemented in the studies: DNA extraction and purification, the PCR technique (two molecular markers), electrophoresis and visualisation of PCR products, and sequencing. A fragment of the beta-giardin gene was used as a molecular marker in order to define the genotype and subgenotype of Giardia. Only in the case of genotyping of two Giardia isolates obtained from Peromyscus eremicus another molecular marker (SSU rRNA)was additionally used. Some widely available computer programmes (Chromas, CAP 3, BioEdit, BLASTn, MEGA version 4.0) were utilised in the analysis of the sequence of the beta-giardin gene fragment and in the phylogenetic analysis. The culture of Giardia trophozoites was established to compare the sequence of the partial beta-giardin gene from cysts and trophozoites. Concentration and purification of Giardia cysts in the saccharose gradient, and the excystation technique were applied in the studies to obtaining an axenic in vitro culture.
