Full Text Available Il volume di anestesia e analgesia locoregionale del cane e del gatto è pensato per essere un testo "da sala operatoria" – come afferma l'autore – perché consente un rapido, ma allo stesso tempo dettagliato, consulto per il libero professionista che si trova a gestire un'anestesia. Si tratta di un piccolo libro, tascabile, che può essere considerato un punto di partenza per gli studenti e per tutti quei medici veterinari che intendono avvicinarsi in maniera specialistica all'arte dell'anestesiologia veterinaria. L'anestesia locoregionale costituisce, infatti, uno strumento insostituibile per la gestione del dolore perioperatorio in medicina veterinaria. Nel volume si forniscono le nozioni di base per praticare i principali blocchi nervosi centrali e periferici. Per ogni blocco sono riportate le tecniche alla cieca (mediante l'ausilio dei punti di repere anatomici e quelle con l'impiego dello stimolatore nervoso periferico. Il volume, corredato da immagini foto e grafici, per un totale di 65 figure, si apre con i capitoli relativi ai farmaci, agli strumenti e alle complicanze dell'anestesia locoregionale. Som det oftest er tilfældet med nye medier, har teenagere være de første til at tage sociale netværkssider til sig. Med denne artikel har jeg til hensigt at beskrive, hvad sociale netværkssider er, samt hvordan de anvendes som en del af ungdomsgenerationens hverdagsliv til vedligeholdelse af sociale relationer. Dette gør jeg ved dels at præsentere en række begreber, som jeg mener er vigtige at forstå i forhold til unges brug af sociale netværk. Og dels vil jeg fremhæve nogle resultater fra min egen forskning i 12-18-årige børn og unges brug af forskellige sociale netværk, heriblandt Arto.com, som længe har været de danske teenageres foretrukne mødested på nettet. Meretoja’s Syndrome: Lattice Corneal Dystrophy, Gelsolin Type

Full Text Available Lattice corneal dystrophy gelsolin type was first described in 1969 by Jouko Meretoja, a Finnish ophthalmologist. It is caused by an autosomal dominant mutation in gelsolin gene resulting in unstable protein fragments and amyloid deposition in various organs. The age of onset is usually after the third decade of life and typical diagnostic triad includes progressive bilateral facial paralysis, loose skin, and lattice corneal dystrophy. We report a case of a 53-year-old female patient referred to our Department of Ophthalmology by severe dry eye and incomplete eyelid closure. She had severe bilateral facial paresis, significant orbicularis, and perioral sagging as well as hypoesthesia of extremities and was diagnosed with Meretoja’s syndrome at the age of 50, confirmed by the presence of gelsolin mutation. At our observation she had bilateral diminished tear film break-up time and Schirmer test, diffuse keratitis, corneal opacification, and neovascularization in the left eye. She was treated with preservative-free lubricants and topical cyclosporine, associated with nocturnal complete occlusion of both eyes, and underwent placement of lacrimal punctal plugs. Ocular symptoms are the first to appear and our role as ophthalmologists is essential for the diagnosis, treatment, and monitoring of ocular alterations in these patients.

The purpose of this paper is to investigate the clinical role of Magnetic Resonance Arthrography (MRA) of the wrist in subjects with chronic pain. Thirty-five patients complaining of wrist pain for more than 6 months were submitted to MRI an MRA. All patients received and intra-articular injection of 2-10 mL of a 10 mmol saline solution of Gd-DPTA. The overall diagnostic accuracy rates of MRI and MRA were 40% and 81% respectively, with sensitivity and specificity of 63% and 39% (MRI) and of 82% and 79% (MRA). The conclusion is that compared with MRI, MRA can be considered a useful tool for the visualization of interosseus carpal ligaments and of the triangular fibrocartilage complex. MRA also helps detect injuries in these structures.

Homicide in the western family and background factors of a perpetrator.

Familicide is a multiple-victim homicide incident in which the killer's spouse and one or more children are slain. A systematic review was conducted to reveal the background factors of western homicide perpetrators. The systematic search was performed in the Arto, Medic, Cinahl, Medline, EBSCOhost Academic Search Premier and Social Services abstracts databases. The keywords were familicide, family homicide, familicide-suicide, filicide-suicide, extended suicide, child, murder, family, filicide and infanticide. The searches revealed 4139 references from the databases. The references were filtered and 32 peer-reviewed research articles revealed in years 2004-2014 were selected as data. The articles were analysed using inductive content analysis, by finding all possible background factors related to homicide. The factors were described as percentages of the range. The background factors of familicide perpetrators were categorised as follows: perpetrators who had committed homicide of a child and intimate partner and possibly committed suicide; a father had who killed a child; a mother who had killed a child; a father who had committed a filicide-suicide; and a mother who had committed a filicide-suicide. Psychological instability, violence and crime were found in all these categories of familicides. Perpetrators who had committed a suicide in addition to the familicide had more often been diagnosed with depression, but they sought treatment for mental health problems less often and had violence and self-destructiveness less often in their background than in other familicide categories. Social and healthcare professionals should be more sensitive to emerging family problems and be prepared for intervention.

The current algebra for second quantized chiral fermions in an external eld contains Schwinger terms. These are studied in two di erent ways. Both are non-perturbative and valid for arbitrary odd dimension of the physical space, although explicit expressions are only given for lower dimensions. The thesis is an introductory text to the four appended research papers. In the rst two papers, Schwinger terms are studied by realizing gauge transformations as linear operators acting on sections of the bundle of Fock spaces parametrized byvector potentials. Bosons and fermions are mixed in a Z2-graded fashion. Charged particles are considered in the rst paper and neutral particles in the second. In the the third and the fourth paper, Schwinger terms are identi ed with cocycles obtained from the family index theorem for a manifold with boundary. A generating form for the covariant anomaly and Schwinger term is obtained in the third paper. The rst three papers consider Yang-Mills while the fourth (in cooperation with Jouko Mickelsson) also includes gravitation. Significant improvement is usually achieved by surgery. However, postoperative pain can be moderate to severe for 2–3 days. The aim of the present study was to evaluate the use of transdermal fentanyl for postoperative pain management after forefoot surgery.Methods: Sixty patients undergoing hallux valgus or hallux rigidus surgery were allocated to receive a patch delivering either fentanyl 12 µg/hour or placebo for postoperative pain. The consumption of rescue opioid oxycodone, the primary outcome measure, was evaluated daily until the fourth postoperative day. Total consumption of oxycodone during the study period was also assessed. Pain scores and possible adverse effects were evaluated every 6 hours during the first 24 hours and on the fourth postoperative day.Results: The use of rescue opioid was low in both groups, the median (range consumption of oxycodone being 10 (0–50 mg on the day of surgery (no difference between the groups, P=0.31 and 0 (0–35 mg thereafter. The total combined consumption was 10 (0–105 mg in the fentanyl group and 20 (0–70 mg in the placebo group (P=0.23. There were no statistically significant differences in pain scores or adverse effects between the groups.Conclusion: As a part of multimodal analgesia with ibuprofen and acetaminophen, a patch delivering fentanyl 12 µg/hour did not significantly decrease the consumption of rescue opioid or pain scores after forefoot surgery

However, some consumers are hesitant to buy remanufactured products because they are skeptical about the quality of the remanufactured product and thus are unsure of the extent to which the product will render services when compared to a new product. A strategy that remanufacturers may employ to entice customers is to offer warranties on remanufactured products. To that end, this paper studies and scrutinizes the impact of offering renewing warranties on remanufactured products. Specifically, the paper suggests a methodology which simultaneously minimizes the cost incurred by the remanufacturers and maximizes the confidence of the consumers towards buying remanufacturing products. Design/methodology/approach: This study uses discrete-event simulation to optimize the implementation of a two-dimensional renewing warranty policy for remanufactured products. The implementation is illustrated using a specific product recovery system called the Advanced Remanufacturing-To-Order (ARTO system. The experiments used in the study were designed using Taguchi’s Orthogonal Arrays to represent the entire domain of the recovery system so as to observe the system behavior under various experimental conditions. In order to determine the optimum strategy offered by the remanufacturer, various warranty and preventive maintenance scenarios were analyzed using pairwise t-tests along with one-way analysis of variance (ANOVA and Tukey pairwise comparisons tests for every scenario. Findings: The proposed methodology is able to simultaneously minimize the cost incurred by the remanufacturer, optimize the warranty price and period, and optimize the preventive maintenance strategy resulting in increased consumer confidence. Professional burnout predicts sick leave and even permanent withdrawal from the labour force. However, knowledge of the barriers to and facilitators of return to work (RTW) in such burnout is limited. To identify factors associated with RTW of burned-out individuals to inform occupational health care (OHC) RTW policy. A systematic search of peer-reviewed quantitative and mixed-method studies published from January 2005 to July 2016 in English and Finnish in ARTO, CINAHL (EBSCO), Medic, PsycINFO (ProQuest), PubMed, Scopus and Web of Science databases, followed by a manual search. We included studies that identify burnout with valid burnout measures and measure the degree of RTW or sick leave as outcomes. We excluded studies with heterogeneous samples without subgroup analyses of RTW in burnout cases. We included 10 studies (three experimental and seven observational) of the initial 1345 identified. The studies reported work-related factors; enhanced communication (positive association) and low control at work (negative association) and individual-related factors; male gender (positive association), covert coping (negative association), high over-commitment to work (positive association) and burnout-related factors; unimpaired sleep (positive association), duration of sick leave over 6 months (negative association) and part-time sick leave (positive association) associated with RTW in burnout. Associations between burnout rehabilitation and RTW, and the level of symptoms and cognitive impairment and RTW remained unclear. Few quantitative studies, of varied methodological quality, explore factors associated with RTW in burnout. Further research is needed to build an evidence base and develop guidelines for supportive OHC actions. © The Author 2017. Optimizing two-dimensional renewable warranty policies for sensor embedded remanufactured products

Remanufactured products, in addition to being environment friendly, are popular with consumers because they can offer the latest technology with lower prices in comparison to brand new products. However, some consumers are hesitant to buy remanufactured products because they are skeptical about the quality of the remanufactured product and thus are unsure of the extent to which the product will render services when compared to a new product. A strategy that remanufacturers may employ to entice customers is to offer warranties on remanufactured products. To that end, this paper studies and scrutinizes the impact of offering renewing warranties on remanufactured products. Specifically, the paper suggests a methodology which simultaneously minimizes the cost incurred by the remanufacturers and maximizes the confidence of the consumers towards buying remanufacturing products. Design/methodology/approach: This study uses discrete-event simulation to optimize the implementation of a two-dimensional renewing warranty policy for remanufactured products. The implementation is illustrated using a specific product recovery system called the Advanced Remanufacturing-To-Order (ARTO) system. The experiments used in the study were designed using Taguchi’s Orthogonal Arrays to represent the entire domain of the recovery system so as to observe the system behavior under various experimental conditions. In order to determine the optimum strategy offered by the remanufacturer, various warranty and preventive maintenance scenarios were analyzed using pairwise t-tests along with one-way analysis of variance (ANOVA) and Tukey pairwise comparisons tests for every scenario. Full Text Available Nguyen X Thanh, Arto Ohinmaa, Charles YanInstitute of Health Economics, Edmonton, Alberta, CanadaObjectives: To estimate the number and cost of asthma-related productivity loss days due to absenteeism and presenteeism (at work but not fully functioning in Alberta in 2005.Methods: Using data from the 2005 Canadian Community Health Survey, this study focused on people of working age (18–64 years, who reported having an asthma diagnosis. Total asthma-related disability days, including in-bed days and activity-restricted days, were estimated by multiplying the difference in the means of total disability days between asthmatics and nonasthmatics adjusted for sociodemographic characteristics and other health conditions by a multiple linear regression, with the number of asthmatics in the population. Number of productivity loss days was a sum between the number of in-bed days (absenteeism and the number of activity-restricted days multiplied by a reduction in functional level (presenteeism, adjusted for five working days per week. Other data from Alberta or Canadian published literature, such as a reduction in functional level of 20%–30%, a labor participation rate of 73%, and an average wage of $158 per day in 2005, were also used for analyses.Results: The prevalence of asthma was estimated at 8.5% among approximately 2.1 million people of working age in Alberta in 2005. The difference in the means of total disability days between asthmatics and nonasthmatics was 0.487 (95% CI: 0.286–0.688 in a period of two weeks or 12.7 (7.5–17.9 in one year. With the reduction in functional level of 20%–30%, the number of asthma-related productivity loss days was estimated from 442 (259–624 to 533 (313–753 thousand, respectively. The corresponding cost was from $70 ($41–$99 to $84 ($49–$119 million. CBT treatment for youth with anxiety and/or depression. This phase will result in a usable app that is ready to be tested for its effectiveness in increasing homework adherence. Phase 3: a pragmatic clinical trial will be conducted at several clinics to evaluate the impact of the app on homework adherence. Participants in the app group are expected to show greater homework completion than those in the no-app group. Phase 3 will be completed by September 2019. universe of issues related to dialogue emerges from thinking perspectives of philosophers as well as communication theorists. On the one hand, the perspective of communication trivializes the phenomenon of dialogue, depriving it of its depth and profoundness; and on the other hand, it defines and specifies the concept of dialogue, assigning to it a form or function. This issue of the journal is devoted to the analysis of the phenomenon of dialogue both in the fields of philosophy and communication, inquiring into different contexts of its development. In her article Communication Solutions by Improving Interactive Art Projects, Gintarė Vainalavičiūtė analyses the relationship between visual arts and contemporary technologies, which determines both the rise of the forms of dialogue and non-traditional understanding of works of art. Mindaugas Stoškus contributed an article entitled Disciplines of Political Philosophy and Political Science: Antagonism, Cooperation or Indifference? in which he investigates the relationship between these two disciplines, conditions and problems pertaining to their dialogue, and the particularly intensified dynamics of the dialogue in the fifties of the 20th century. In their article Online Artistic Activism: Case-Study of Hungarian-Romanian Intercultural Communication, Gizela Horváth and Rozália Klára Bakó delve into the interactive relationship between works of art and their perceiver, as these works of art send messages via the social media environment. Moral Perception, Cognition, and Dialogue is an article authored by Vojko Strahovnik, in which he examines the causes for the rise of cases that hinder intercommunication and mutual understanding, such as disagreement, intercultural dialogues, etc. Problems of visual communication and the specificity of visual languages, bringing together subjects into dialogue are discussed by Arto Mutanen in his article Relativity of Visual Communication. the Baltic and the Slavic languages were descended from the different dialects of the north area of Indo-European parent language and only a bit later the period of close contacts of Proto-Baltic and Proto-Slavic came. There are some inovations in Baltic, Slavic and Germanic, which came of on the period, when thees three groups of languages were not separated from the north dialect area of Indo-European. One of thees inovations is sugested to be the suffix *- īti̯o - (<*-īto- + -i̯o-, the oldest function of which is the formation of nomina atributiva (cf. Lith. ausỹtė “wintry cap with ear-tabs"”, Latv. lēnītis “warm south or west wind”, South Slav. golit’ь “naked, poor man”. Through the process of the meaning specialization the derivatives with suffix *- īti̯o - got the meaning of nomina deminutiva in Baltic (cf. Lith. sūnỹtis, Latv. sūnītis “a little son”, the meaning of nomina patronimica and originis in Slavic (cf. Slav, bratit'ь “a brothes son”, slobodit’ь  “countryman from a free land” and the meaning of nomina collectiva in Germanic (cf. MHG. geveterīde “parents”. Later on the period of close contacts of Proto-Baltic and Proto-Slavic languages some derivatives with this suffix got also the meaning of nomina patroninica and nomina originis in Baltic dialects (especialy in West Baltic, cf. Lith. dial, vókytis, Latv. vācītis “German” from Proto-Slavic and some of them got the meaning of nomina deminutiva in South Slavic languages (cf. vl̥’čit’ь „a little wolf-cub” from Proto-Baltic. There are some old inovations which came of in Proto-Baltic and later feld under the Proto-Slavic language. In this way the development of nomina agentis with suffixes *- tāi̯o- (cf. Full Text Available Este trabajo tuvo como objetivo central reconstruir las condiciones climáticas y la vegetaci6n durante los últimos 4.000 años en el norte de la cordillera Occidental de Colombia con base en dos perforaciones realizadas en los sectores Llano Grande y Puente Largo del paramo de Frontino y su correlación con otras perforaciones llevadas a cabo en el mismo paramo y en el macizo de Tatama. La buena resolución metodológica empleada (elección adecuada de los sitios de muestreo, sonda que no compacta los sedimentos, submuestreo al centímetro y conteo de alícuotas, acetolisis suave y buen numero de radiodataciones hizo posible detectar y caracterizar tanto en duración como en amplitud, cambios en la vegetación y el clima cuyo registro en el trópico es objeto de debate. En general los períodos mas fríos fueron húmedos (vegetación regional dominante: paramo; local: acuáticas y de pantano y los mas cálidos secos (vegetación regional dominante: bosque; local: cojines. Los principales cambios climáticos ocurridos en estos cuatro milenios fueron los siguientes: La transición entre el Holoceno Medio y Tardío (4.000-3.200 arias interpol. I•C BP fue la fase mas húmeda, con pulsos menos húmedos y cálidos alrededor de 3.900, 3.800, 3.500-3.600 y 3.300-3.200 años interpol. I•C BP. El período comprendido entre 3.200-2.650 años interpol.
