    The activity of the epidermal growth factor receptor (EGFR)-directed monoclonal antibody cetuximab combined with oxaliplatin/leucovorin/5-fluorouracil (FUFOX) was assessed in first-line metastatic gastric and oesophago-gastric junction (OGJ) cancer in a prospective phase II study showing a promising objective tumour response rate of 65% and a low mutation frequency of KRAS (3%). The aim of the correlative tumour tissue studies was to investigate the relationship between EGFR gene copy numbers, activation of the EGFR pathway, expression and mutation of E-cadherin, V600E BRAF mutation and clinical outcome of patients with gastric and OGJ cancer treated with cetuximab combined with FUFOX. Patients included in this correlative study (n = 39) were a subset of patients from the clinical phase II study. The association between EGFR gene copy number, activation of the EGFR pathway, abundance and mutation of E-cadherin which plays an important role in these disorders, BRAF mutation and clinical outcome of patients was studied. EGFR gene copy number was assessed by FISH. Expression of the phosphorylated forms of EGFR and its downstream effectors Akt and MAPK, in addition to E-cadherin was analysed by immunohistochemistry. The frequency of mutant V600E BRAF was evaluated by allele-specific PCR and the mutation profile of the E-cadherin gene CDH1 was examined by DHPLC followed by direct sequence analysis. Correlations with overall survival (OS), time to progression (TTP) and overall response rate (ORR) were assessed. Our study showed a significant association between increased EGFR gene copy number (≥ 4.0) and OS in gastric and OGJ cancer, indicating the possibility that patients may be selected for treatment on a genetic basis. Furthermore, a significant correlation was shown between activated EGFR and shorter TTP and ORR, but not between activated EGFR and OS. No V600E BRAF mutations were identified. On the other hand, an interesting trend between high E-cadherin expression levels and better OS was observed and two CDH1 exon 9 missense mutations (A408V and D402H) were detected.     Surgical resection has a potential benefit for patients with metastatic adenocarcinoma of the stomach and gastroesophageal junction. To evaluate outcome in patients with limited metastatic disease who receive chemotherapy first and proceed to surgical resection. The AIO-FLOT3 (Arbeitsgemeinschaft Internistische Onkologie-fluorouracil, leucovorin, oxaliplatin, and docetaxel) trial is a prospective, phase 2 trial of 252 patients with resectable or metastatic gastric or gastroesophageal junction adenocarcinoma. Patients were enrolled from 52 cancer care centers in Germany between February 1, 2009, and January 31, 2010, and stratified to 1 of 3 groups: resectable (arm A), limited metastatic (arm B), or extensive metastatic (arm C). Data cutoff was January 2012, and the analysis was performed in March 2013. Patients in arm A received 4 preoperative cycles of fluorouracil, leucovorin, oxaliplatin, and docetaxel (FLOT) followed by surgery and 4 postoperative cycles. Patients in arm B received at least 4 cycles of neoadjuvant FLOT and proceeded to surgical resection if restaging (using computed tomography and magnetic resonance imaging) showed a chance of margin-free (R0) resection of the primary tumor and at least a macroscopic complete resection of the metastatic lesions. Patients in arm C were offered FLOT chemotherapy and surgery only if required for palliation. Patients received a median (range) of 8 (1-15) cycles of FLOT. The primary end point was overall survival. In total, 238 of 252 patients (94.4%) were eligible to participate. The median (range) age of participants was 66 (36-79) years in arm A (n = 51), 63 (28-79) years in arm B (n = 60), and 65 (23-83) years in arm C (n = 127). Patients in arm B (n = 60) had only retroperitoneal lymph node involvement (27 patients [45%]), liver involvement (11 [18.3%]), lung involvement (10 [16.7%]), localized peritoneal involvement (4 [6.7%]), or other (8 [13.3%]) incurable sites.     Full Text Available All-in-one admixtures (AIO-admixtures provide safe, effective and low-risk PN (parenteral nutrition for practically all indications and applications. Water, energy (carbohydrates and lipids, amino acids, vitamins and trace elements are infused together with PN either as industrially-manufactured AIO admixtures provided as two- or three-chamber bags (shelf life usually more than 12 months completed with electrolytes and micronutrients where appropriate or as individually compounded ready-to-use AIO admixtures (compounding, usually prepared by a pharmacy on either a daily or weekly basis and stored at 2–8°C. Physico-chemical and microbial stability of an AIO admixture is essential for the safety and effectiveness of patient-specific PN, and its assurance requires specialist pharmaceutical knowledge. The stability should be documented for an application period of 24 (–48 hours. It is advisable to offer a limited selection of different PN regimes in each hospital. For reasons of drug and medication safety, PN admixtures prepared for individual patients must be correctly labelled and specifications for storage conditions must also be followed during transport. Monitoring is required where applicable. Micronutrients are usually administered separately to AIO admixtures. In case compatibility and stability have been well documented trace elements and/or combination preparations including water-soluble or water-soluble/fat soluble vitamin supplements can be added to PN admixtures under strict aseptic conditions.     To date, only limited toxicity data are available for the combination of erlotinib with either capecitabine or gemcitabine as front-line therapy for advanced pancreatic cancer. Within a randomized phase III trial, 281 treatment-naive patients were randomly assigned between capecitabine (2000 mg/m/day, for 14 days, once every 3 weeks) plus erlotinib (150 mg/day, arm A) and gemcitabine (1000 mg/m as a 30-min infusion) plus erlotinib (150 mg/day, arm B). In case of treatment failure, patients were crossed over to a second-line treatment with the comparator cytostatic drug without erlotinib. The primary study endpoint was the time to treatment failure of second-line therapy (TTF2). This interim analysis of toxicity contains safety data from the first 127 randomized patients. During first-line therapy, patients received a median number of three treatment cycles (range 0-13) in both the arms. Regarding chemotherapy, a treatment delay was observed in 12% of the cycles in arm A and in 22% of the cycles in arm B. Dose reductions of the cytostatic drug were performed in 18 and 27% of treatment cycles, respectively. Erlotinib dose reductions were performed in 6 and 11% of all cycles. Grade 3/4 hematological toxicity was arms; major grade 3/4 toxicities in arms A and B were diarrhea (9 vs. 7%), skin rash (4 vs. 12%), and hand-foot syndrome (7 vs. 0%). No treatment-related death was observed. In conclusion, this interim safety analysis suggests that treatment with erlotinib 150 mg/day is feasible in combination with capecitabine or gemcitabine.

  6. Diversity and abundance of the arsenite oxidase gene aioA in geothermal areas of Tengchong, Yunnan, China.

    A total of 12 samples were collected from the Tengchong geothermal areas of Yunnan, China, with the goal to assess the arsenite (AsIII) oxidation potential of the extant microbial communities as inferred by the abundance and diversity of the AsIII oxidase large subunit gene aioA relative to geochemical context. Arsenic concentrations were higher (on average 251.68 μg/L) in neutral or alkaline springs than in acidic springs (on average 30.88 μg/L). aioA abundance ranged from 1.63 × 10(1) to 7.08 × 10(3) per ng of DNA and positively correlated with sulfide and the ratios of arsenate (AsV):total dissolved arsenic (AsTot). Based on qPCR estimates of bacterial and archaeal 16S rRNA gene abundance, aioA-harboring organisms comprised as much as ~15% of the total community. Phylogenetically, the major aioA sequences (270 total) in the acidic hot springs (pH 3.3-4.4) were affiliated with Aquificales and Rhizobiales, while those in neutral or alkaline springs (pH 6.6-9.1) were inferred to be primarily bacteria related to Thermales and Burkholderiales. Interestingly, aioA abundance at one site greatly exceeded bacterial 16S rRNA gene abundance, suggesting these aioA genes were archaeal even though phylogenetically these aioA sequences were most similar to the Aquificales. In summary, this study described novel aioA sequences in geothermal features geographically far removed from those in the heavily studied Yellowstone geothermal complex.

    This paper has a research on resource management and scheduling policy based on grid technology for Agricultural Internet of Things (AIoT). Facing the situation of a variety of complex and heterogeneous agricultural resources in AIoT, it is difficult to represent them in a unified way. But from an abstract perspective, there are some common models which can express their characteristics and features. Based on this, we proposed a high-level model called Agricultural Resource Hierarchy Model (ARHM), which can be used for modeling various resources. It introduces the agricultural resource modeling method based on this model. Compared with traditional application-oriented three-layer model, ARHM can hide the differences of different applications and make all applications have a unified interface layer and be implemented without distinction. Furthermore, it proposes a Web Service Resource Framework (WSRF)-based resource management method and the encapsulation structure for it. Finally, it focuses on the discussion of multi-agent-based AG resource scheduler, which is a collaborative service provider pattern in multiple agricultural production domains.

    Diagnostic imaging plays an essential role in pediatric oncology with regard to diagnosis, therapy-planning, and the follow-up of solid tumors. The current imaging standard in pediatric oncology includes a variety of radiological and nuclear medicine imaging modalities depending on the specific tumor entity. The introduction of combined simultaneous positron emission tomography (PET) and magnetic resonance imaging (MRI) has opened up new diagnostic options in pediatric oncology. This novel modality combines the excellent anatomical accuracy of MRI with the metabolic information of PET. In initial clinical studies, the technical feasibility and possible diagnostic advantages of combined PET-MRI have been in comparison with alternative imaging techniques. It was shown that a reduction in radiation exposure of up to 70 % is achievable compared with PET-CT. Furthermore, it has been shown that the number of imaging studies necessary can be markedly reduced using combined PET-MRI. Owing to its limited availability, combined PET-MRI is currently not used as a routine procedure. However, this new modality has the potential to become the imaging reference standard in pediatric oncology in the future. This review article summarizes the central aspects of pediatric oncological PET-MRI based on existing literature. Typical pediatric oncological PET-MRI cases are also presented. (orig.)     The traditional three-layer Internet of things (IoT) model, which includes physical perception layer, information transferring layer and service application layer, cannot express complexity and diversity in agricultural engineering area completely. It is hard to categorize, organize and manage the agricultural things with these three layers. Based on the above requirements, we propose a new service-oriented grid-based method to set up and build the agricultural IoT. Considering the heterogeneous, limitation, transparency and leveling attributes of agricultural things, we propose an abstract model for all agricultural resources. This model is service-oriented and expressed with Open Grid Services Architecture (OGSA). Information and data of agricultural things were described and encapsulated by using XML in this model. Every agricultural engineering application will provide service by enabling one application node in this service-oriented grid. Description of Web Service Resource Framework (WSRF)-based Agricultural Internet of Things (AIoT) and the encapsulation method were also discussed in this paper for resource management in this model.

    This multicentric, randomized, two-stage phase II trial evaluated three simplified weekly infusional regimens of fluorouracil (FU) or FU plus folinic acid (FA) and cisplatin (Cis) with the aim to select a regimen for future phase III trials. A total of 145 patients with advanced gastric cancer where randomly assigned to weekly FU 3,000 mg/m2/24 hours (HD-FU), FU 2,600 mg/m2/24 hours plus dl-FA 500 mg/m2 or l-FA 250 mg/m2 (HD-FU/FA), or FU 2000 mg/m2/24 hours plus FA plus biweekly Cis 50 mg/m2, each administered for 6 weeks with a 1-week rest. The primary end point was the response rate. Confirmed responses were observed in 6.1% (two of 33) of the eligible patients treated with HD-FU, in 25% (12 of 48, including one complete remission [CR]) with HD-FU/FA, and in 45.7% (21 of 46, including four CRs) with HD-FU/FA/Cis. The HD-FU arm was closed after stage 1 because the required minimum number of responses was not met. The median progression-free survival of all patients in the HD-FU, HD-FU/FA, and HD-FU/FA/Cis arm was 1.9, 4.0, and 6.1 months, respectively. The median overall survival was 7.1, 8.9, and 9.7 months, and the survival rate at 1 year was 24.3%, 30.3%, and 45.3%, respectively. Grade 4 toxicities were rare. The most relevant grade 3/4 toxicities were neutropenia in 1.9%, 5.4%, and 19.6%, and diarrhea in 2.7%, 1.9%, and 3.9% of the cycles in the HD-FU, HD-FU/FA, and HD-/FU/Cis arms, respectively. Weekly infusional FU/FA plus biweekly Cis is effective and safe in patients with gastric cancer.

  14. The Sump Screen Clogging Issue in Belgium from the Standpoint of the Authorized Inspection Organisation (AIO) (AVN)

    All Belgian NPPs are PWRs. These PWRs have different containment designs and different sump designs. The four most recent units took RG 1.82 rev. 0 into account for the recirculation sump design. Several improvements were made in the framework of the first Periodic Safety Review to the oldest units such as enlargement of the sumps strainer in 1985, taking into account the requirements of RG 1.82 rev. 0. The problem of recirculation sump screen clogging due to the accumulation of insulation debris after an accident was also identified as a safety issue in the first Periodic Safety Review for Doel 3 and Tihange 2 and the second Periodic Safety Review for Doel 1 and 2 and Tihange 1, that started in the nineties. One of the purposes of the Periodic Safety Review being the justification of the safety level of the power plants with regard to the most recent safety rules and practices, the licensee was asked to review the characteristics of the sump strainers according to the revision 1 of the RG 1.82 (which referred to Nureg 0897 rev. 1). Later on, the Barsebaeck incident and the events leading to the NRC IN 93-34 showed that the conclusions of Nureg 0897 could underestimate the potential for loss of NPSH of the ECCS- and CSS-pumps. The issue was further examined and the AIO asked the licensee to continue efforts to recollect information. After issuance of the results from the parametric evaluation for pressurised water reactor recirculation sump performance (GSI-191) performed by LANL under NRC contract, the AIO asked the Belgian licensee in August 2002 to increase its efforts to investigate and to solve this issue. The requirements to the Licensee and the corresponding action plan as approved by the AIO are presented. Some significant lessons learned from the implementation of this action plan are discussed.   15. Effects upon metabolic pathways and energy production by Sb(III and As(III/Sb(III-oxidase gene aioA in Agrobacterium tumefaciens GW4.

    Full Text Available Agrobacterium tumefaciens GW4 is a heterotrophic arsenite [As(III]/antimonite [Sb(III]-oxidizing strain. The As(III oxidase AioAB is responsible for As(III oxidation in the periplasm and it is also involved in Sb(III oxidation in Agrobacterium tumefaciens 5A. In addition, Sb(III oxidase AnoA and cellular H2O2 are also responsible for Sb(III oxidation in strain GW4. However, the deletion of aioA increased the Sb(III oxidation efficiency in strain GW4. In the present study, we found that the cell mobility to Sb(III, ATP and NADH contents and heat release were also increased by Sb(III and more significantly in the aioA mutant. Proteomics and transcriptional analyses showed that proteins/genes involved in Sb(III oxidation and resistance, stress responses, carbon metabolism, cell mobility, phosphonate and phosphinate metabolism, and amino acid and nucleotide metabolism were induced by Sb(III and were more significantly induced in the aioA mutant. The results suggested that Sb(III oxidation may produce energy. In addition, without periplasmic AioAB, more Sb(III would enter bacterial cells, however, the cytoplasmic AnoA and the oxidative stress response proteins were significantly up-regulated, which may contribute to the increased Sb(III oxidation efficiency. Moreover, the carbon metabolism was also activated to generate more energy against Sb(III stress. The generated energy may be used in Sb transportation, DNA repair, amino acid synthesis, and cell mobility, and may be released in the form of heat.

    Purpose: The impact of the delivery of radiotherapy (RT) on treatment results in rectal cancer patients is unknown. Methods and Materials: The data from 788 patients with rectal cancer treated within the German CAO/AIO/ARO-94 phase III trial were analyzed concerning the impact of the delivery of RT (adequate RT: minimal radiation RT dose delivered, 4300 cGy for neoadjuvant RT or 4700 cGy for adjuvant RT; completion of RT in <44 days for neoadjuvant RT or <49 days for adjuvant RT) in different centers on the locoregional recurrence rate (LRR) and disease-free survival (DFS) at 5 years. The LRR, DFS, and delivery of RT were analyzed as endpoints in multivariate analysis. Results: A significant difference was found between the centers and the delivery of RT. The overall delivery of RT was a prognostic factor for the LRR (no RT, 29.6% ± 7.8%; inadequate RT, 21.2% ± 5.6%; adequate RT, 6.8% ± 1.4%; p = 0.0001) and DFS (no RT, 55.1% ± 9.1%; inadequate RT, 57.4% ± 6.3%; adequate RT, 69.1% ± 2.3%; p = 0.02). Postoperatively, delivery of RT was a prognostic factor for LRR on multivariate analysis (together with pathologic stage) but not for DFS (independent parameters, pathologic stage and age). Preoperatively, on multivariate analysis, pathologic stage, but not delivery of RT, was an independent prognostic parameter for LRR and DFS (together with adequate chemotherapy). On multivariate analysis, the treatment center, treatment schedule (neoadjuvant vs. adjuvant RT), and gender were prognostic parameters for adequate RT.     Preoperative chemoradiotherapy with infusional fluorouracil, total mesorectal excision surgery, and postoperative chemotherapy with fluorouracil was established by the German CAO/ARO/AIO-94 trial as a standard combined modality treatment for locally advanced rectal cancer. Here we compare the previously established regimen with an investigational regimen in which oxaliplatin was added to both preoperative chemoradiotherapy and postoperative chemotherapy. In this multicentre, open-label, randomised, phase 3 study we randomly assigned patients with rectal adenocarcinoma, clinically staged as cT3-4 or any node-positive disease, to two groups: a control group receiving standard fluorouracil-based combined modality treatment, consisting of preoperative radiotherapy of 50·4 Gy in 28 fractions plus infusional fluorouracil (1000 mg/m(2) on days 1-5 and 29-33), followed by surgery and four cycles of bolus fluorouracil (500 mg/m(2) on days 1-5 and 29); or to an investigational group receiving preoperative radiotherapy of 50·4 Gy in 28 fractions plus infusional fluorouracil (250 mg/m(2) on days 1-14 and 22-35) and oxaliplatin (50 mg/m(2) on days 1, 8, 22, and 29), followed by surgery and eight cycles of oxaliplatin (100 mg/m(2) on days 1 and 15), leucovorin (400 mg/m(2) on days 1 and 15), and infusional fluorouracil (2400 mg/m(2) on days 1-2 and 15-16). Randomisation was done with computer-generated block-randomisation codes stratified by centre, clinical T category (cT1-3 vs cT4), and clinical N category (cN0 vs cN1-2) without masking.     Partly because of its rising incidence, but mostly because of the availability of modern examination techniques, the detection rate of small renal-cell carcinomas is increasing more and more. Even though tumors exceeding 4 cm in diameter rarely metastasize, all renal lesions that arouse the suspicion of malignancy should be treated. Operative treatment techniques such as radical and partial nephrectomy are increasingly carried out as laparoscopic procedures and are regarded as the gold standard. Modern thermal ablation techniques may be a helpful treatment option for patients who are unfit for a surgical resection or refuse it. Radiofrequency ablation (RFA) is the most frequently applied of these methods. Modern probes allow ablation of lesions between 2 and 5 cm in diameter. In the vast majority of cases RFA is carried out percutaneously and monitored by ultrasound or CT; it is deemed safe and the complication rate is low. While randomized comparative studies against open resection are yet available, the preliminary results obtained with renal RFA are promising and suggest that RFA may be superior to other thermal ablation techniques. Clinical success rates are over 90% for radiofrequency, and also for cryotherapy, which is performed less often. Local relapse is very uncommon. (orig.) Alle tumorsuspekten Veraenderungen gelten als behandlungspflichtig, auch wenn Nierenzellkarzinome unter 4 cm Groesse sehr selten metatastasieren. Als Goldstandard gelten die operativen Behandlungsverfahren wie die radikale oder partielle Nephrektomie, die zunehmend auch laparoskopisch durchgefuehrt werden. Bei Kontraindikationen zur Operation oder bei Patienten, die eine Operation ablehnen, koennen moderne Thermoablationsverfahren eine Therapiealternative sein. Die Radiofrequenzablation (RFA) ist das von allen am haeufigsten angewendete Verfahren. Moderne Sondentypen ermoeglichen Laesionsgroessen zwischen 2 und 5 cm. Die RFA wird ueberwiegend perkutan unter Ultraschall- oder CT-Kontrolle vorgenommen, gilt als sicher und komplikationsarm. Wenngleich randomisierte Vergleichsstudien zu operativen Verfahren noch immer fehlen, sind die bisherigen Erfahrungen gut und lassen die RFA gegenueber den anderen thermischen Verfahren vorteilhaft erscheinen. Fuer die RFA sowie die seltener angewendete Kryotherapie betragen die klinischen Erfolgsraten ueber 90%; Tumorrezidive sind sehr selten. (orig.)

    Pancreatic cancer is a highly malignant disease and despite progress in systemic therapy survival is still short. For patients with R0/R1 resected disease, adjuvant chemotherapy with gemcitabine has been established as the standard treatment. More controversy exists with regard to optimal treatment of locally advanced non-metastatic pancreatic cancer. However, there is evidence to suggest that patients who respond to an initial phase of chemotherapy may be those who benefit most from sequential chemoradiotherapy. Specifically, in the treatment of advanced and metastatic disease chemotherapy or radiotherapy cannot stand alone but must be accompanied by multidisciplinary treatment approaches involving pain management, weight control, psychooncological care and palliative care. Monotherapy with gemcitabine and the combined use of gemcitabine with erlotinib are established standards for treatment of metastatic pancreatic cancer. Patients in a good general condition but where the gemcitabine-based therapy failed should be offered second-line treatment. (orig.) [German] Das Pankreaskarzinom ist eine hochmaligne Erkrankung, die trotz nachweisbarer klinischer Fortschritte weiterhin mit einem meist kurzen Ueberleben verbunden ist. Bei R0/R1-resezierten Patienten gilt eine adjuvante Behandlung mit Gemcitabin gegenwaertig als etablierter Behandlungsstandard. Weniger eindeutig ist die optimale Therapie des lokal fortgeschrittenen, nichtmetastasierten Pankreaskarzinoms (LAPC). Es gibt aber Hinweise dafuer, dass Patienten, die auf eine initiale Chemotherapiephase ansprachen, von einer nachgeschalteten Radiochemotherapie profitieren koennen. Gerade zur Behandlung des metastasierten Pankreaskarzinoms sollten die Chemo- oder die Radiochemotherapie nicht allein stehen, sondern durch multidisziplinaere Behandlungsansaetze unterstuetzt werden. Dazu gehoeren die Schmerz- und Ernaehrungstherapie, Psychoonkologie und Palliativmedizin. Bei der Behandlung des metastasierten Pankreaskarzinoms gehoeren die Gemcitabin-Monotherapie sowie auch die Kombination aus Gemcitabin und Erlotinib zu den etablierten Standards. Nach Versagen einer Gemcitabin-basierten Therapie sollte Patienten in gutem Allgemeinzustand eine Zweitlinientherapie angeboten werden. (orig.)

    Approximately one-third of all borderline ovarian tumours (BOT) are diagnosed in patients with child-bearing potential. Detailed information regarding their specific characteristics and prognostic factors is limited. Clinical parameters of BOT patients treated between 1998 and 2008 in 24 German centres were retrospectively investigated. Central pathology review and prospective follow-up were carried out. Patients BOT patients with a median age of 49.1 (14.1-91.5) years were analysed [280 patients BOT patients with child-bearing potential are at higher risk for disease recurrence. However, relapses usually remain BOT in the preserved ovaries as opposed to older patients being at higher risk for malignant transformation in peritoneal or distant localisation. Therefore, fertility-sparing approach can be justified for younger patients after thorough consultation. © The Author 2014. A total of 18 participants, nine patients diagnosed with advanced lung cancer (International Classification of Diseases, tenth edition, diagnosis C-34, stage IV starting or receiving palliative treatment and nine relatives, were interviewed. Data were interpreted through qualitative content analysis.Results: We identified four main categories of difficulties: communication and conflicts, home and everyday life, thinking about cancer, and treatment trajectory.     Patients with locally advanced operable rectal cancer (uT3/4 or uN+, Mason CS III/IV) were randomly assigned to pre- or postoperative radiochemotherapy: A total dose of 50.4 Gy (single dose 1.8 Gy) was applied to the tumor and the pelvic lymph nodes. 5-FU (1,000 mg/m 2 /d) was administered concomitantly in the first and fifth week of radiation as 120-h continuous infusion. Four additional cycles of 5-FU chemotherapy (500 mg/m 2 d, iv bolus) were applied. Radiochemotherapy was identical in both arms except for a small-volume boost of 5.4 Gy in the postoperative setting. Time interval between radiochemotherapy and surgery was 4-6 weeks in both arms. Techniques of surgery were standardized and included total mesorectal excision. In addition, stratification according to surgeons involved has been provided for. Primary endpoints of the study are 5-year overall-survival, local and distant control, secondary endpoints include rate of curative (R0) resections and sphincter saving procedures, toxicity of radiochemotherapy, surgical complications and quality of life. Results: As of 15th November 2000, 628 patients were randomized from 26 participating institutions: 310 patients were randomized to postoperative radiochemotherapy, 318 patients to preoperative radiochemotherapy. Acute toxicity (WHO) of radiochemotherapy was low, with less than 15% of patients experiencing Grade 3 or higher toxicity: The principal toxicity was diarrhea, with 12% in the postoperative radiochemotherapy arm and 10% in the preoperative radiochemotherapy arm having Grade-3, and 1% in either arm having Grade-4 diarrhea. Erythema, nausea and leukopenia were the next common toxicities, with less than 3% of patients in either arm suffering Grade 3 or greater leukopenia or nausea. Postoperative complication rates were similar in both arms, with 12% (postoperative radiochemotherapy) and 13% (preoperative radiochemotherapy) of patients, respectively, suffering from anastomotic leakage, 4% (postoperative radiochemotherapy) and 3% (preoperative radiochemotherapy) from postoperative bleeding, and 6% (postoperative radiochemotherapy) and 5% (preoperative radiochemotherapy) from delayed wound healing. Conclusion: The patient accrual of our trial is satisfactory, neoadjuvant radiochemotherapy is well tolerated and bears no higher risk for postoperative morbidity. (orig.) [de

    New treatment options for patients with metastatic Soft Tissue Sarcoma are urgently needed. Preclinical studies suggested activity of vorinostat, a histone deacetylase inhibitor. A multi-centre, open-label, non-randomised phase II trial to investigate the efficacy and safety of vorinostat in patients with locally advanced or metastatic Soft Tissue Sarcoma failing 1st-line anthracycline-based chemotherapy was initiated. Patients were treated with vorinostat 400 mg po qd for 28 d followed by a treatment-free period of 7 d, representing a treatment cycle of 5 weeks. Restaging was performed every three cycles or at clinical progression. Between 06/10 and 09/13, 40 Soft Tissue Sarcoma patients were treated with vorinostat at seven participating centres. Patients had received 1 (n=8, 20%), 2 (n=10, 25%) or ≥3 (n=22, 55%) previous lines of chemotherapy. Best response after three cycles of treatment was stable disease (n=9, 23%). Median progression-free survival and overall survival were 3.2 and 12.3 months, respectively. Six patients showed long-lasting disease stabilisation for up to ten cycles. Statistical analyses failed to identify baseline predictive markers in this subgroup. Major toxicities (grade ≥III) included haematological toxicity (n=6, 15%) gastrointestinal disorders (n=5, 13%), fatigue (n=4, 10%), musculoskeletal pain (n=4, 10%), and pneumonia (n=2, 5%). In a heavily pre-treated patient population, objective response to vorinostat was low. However, a small subgroup of patients had long-lasting disease stabilisation. Further studies aiming to identify predictive markers for treatment response as well as exploration of combination regimens are warranted.     Importance Surgical resection has a potential benefit for patients with metastatic adenocarcinoma of the stomach and gastroesophageal junction. Objective To evaluate outcome in patients with limited metastatic disease who receive chemotherapy first and proceed to surgical resection. Design, Setting, and Participants The AIO-FLOT3 (Arbeitsgemeinschaft Internistische Onkologie–fluorouracil, leucovorin, oxaliplatin, and docetaxel) trial is a prospective, phase 2 trial of 252 patients with resectable or metastatic gastric or gastroesophageal junction adenocarcinoma. Patients were enrolled from 52 cancer care centers in Germany between February 1, 2009, and January 31, 2010, and stratified to 1 of 3 groups: resectable (arm A), limited metastatic (arm B), or extensive metastatic (arm C). Data cutoff was January 2012, and the analysis was performed in March 2013. Interventions Patients in arm A received 4 preoperative cycles of fluorouracil, leucovorin, oxaliplatin, and docetaxel (FLOT) followed by surgery and 4 postoperative cycles. Patients in arm B received at least 4 cycles of neoadjuvant FLOT and proceeded to surgical resection if restaging (using computed tomography and magnetic resonance imaging) showed a chance of margin-free (R0) resection of the primary tumor and at least a macroscopic complete resection of the metastatic lesions. Patients in arm C were offered FLOT chemotherapy and surgery only if required for palliation. Patients received a median (range) of 8 (1-15) cycles of FLOT. Main Outcomes and Measures The primary end point was overall survival. Results In total, 238 of 252 patients (94.4%) were eligible to participate. The median (range) age of participants was 66 (36-79) years in arm A (n = 51), 63 (28-79) years in arm B (n = 60), and 65 (23-83) years in arm C (n = 127).     The genetic organization of the aioBA operon, encoding the arsenite oxidase of the moderately acidophilic and facultative chemoautotrophic bacterium Thiomonas arsenitoxydans, is different from that of the aioBA operon in the other arsenite oxidizers, in that it encodes AioF, a metalloprotein belonging to the ArsR/SmtB family. AioF is stabilized by arsenite, arsenate, or antimonite but not molybdate. Arsenic is tightly attached to AioF, likely by cysteine residues. When loaded with arsenite or arsenate, AioF is able to bind specifically to the regulatory region of the aio operon at two distinct positions. In Thiomonas arsenitoxydans, the promoters of aioX and aioB are convergent, suggesting that transcriptional interference occurs. These results indicate that the regulation of the aioBA operon is more complex in Thiomonas arsenitoxydans than in the other aioBA containing arsenite oxidizers and that the arsenic binding protein AioF is involved in this regulation. On the basis of these data, a model to explain the tight control of aioBA expression by arsenic in Thiomonas arsenitoxydans is proposed. Copyright © 2014, American Society for Microbiology.     Historical data indicate that surgical resection may benefit select patients with metastatic gastric and gastroesophageal junction cancer. However, randomized clinical trials are lacking. The current RENAISSANCE trial addresses the potential benefits of surgical intervention in gastric and gastroesophageal junction cancer with limited metastases. This is a prospective, multicenter, randomized, investigator-initiated phase III trial. Previously untreated patients with limited metastatic stage (retroperitoneal lymph node metastases only or a maximum of one incurable organ site that is potentially resectable or locally controllable with or without retroperitoneal lymph nodes) receive 4 cycles of FLOT chemotherapy alone or with trastuzumab if Her2+. Patients without disease progression after 4 cycles are randomized 1:1 to receive additional chemotherapy cycles or surgical resection of primary and metastases followed by subsequent chemotherapy. 271 patients are to be allocated to the trial, of which at least 176 patients will proceed to randomization. The primary endpoint is overall survival; main secondary endpoints are quality of life assessed by EORTC-QLQ-C30 questionnaire, progression free survival and surgical morbidity and mortality. Recruitment has already started; currently (Feb 2017) 22 patients have been enrolled. If the RENAISSANCE concept proves to be effective, this could potentially lead to a new standard of therapy. On the contrary, if the outcome is negative, patients with gastric or GEJ cancer and metastases will no longer be considered candidates for surgical intervention.     Hybrid imaging of function and morphology has gained significant importance for lesion detection and treatment monitoring in oncology. In patients with differentiated thyroid carcinoma (DTC) a planar whole body scan is carried out after radioiodine therapy (RIT) for staging. However, due to limited spatial resolution the diagnostic accuracy of this scintigraphy method is impaired. Radioembolization utilizing {sup 90}Yttrium loaded micro-spheres by selective internal radiotherapy (SIRT) allows a minor invasive therapy of primary and secondary liver tumors. In order to avoid side effects of the micro-spheres caused by an outflow into intestines, stomach or lungs, imaging the arteries supplying the liver has to be performed by means of technetium-99m macroaggregated albumin ({sup 99m}Tc-MAA) and scintigraphy. The limited morphological information supplied by scintigraphy is again a challenge in treatment monitoring. {sup 131}Iodine whole body scanning is used for staging in patients with DTC 3-4 days after ablation. Monitoring of the tumor marker thyroglobulin and selective radioiodine whole body scans are available for patients with a high risk profile in the further follow-up with imaging of the arteries supplying the liver by means of {sup 99m}Tc-MAA scintigraphy in preparation of SIRT. Single photon emission computed tomography/computed tomography (SPECT/CT) of the neck and thorax with a therapeutic activity of radioiodine for staging after ablation. Techniques include imaging of arteries supplying the liver by means of {sup 99m}Tc-MAA SPECT/CT before SIRT and evaluation and quantification of the uptake of liver tumors, especially in comparison to the uptake of liver parenchyma by means of SPECT/CT. Due to the integration of combined functional and morphological information SPECT/CT can be used to characterize the morphology and iodine uptake of lesions more accurately, resulting in optimized staging in patients with DTC in comparison to whole body iodine scans and SPECT/CT provides more accurate imaging of the arterial supply of the liver and of potential outflows of micro-spheres into other organs. SPECT/CT allows evaluation and quantification of the uptake of liver tumors. Improved postablative staging in patients with differentiated thyroid cancer by SPECT/CT in comparison to radioiodine whole body scans can be achieved. Improved planning and monitoring of SIRT therapies utilizing SPECT/CT leads to optimized therapeutic doses within liver lesions. Integration of SPECT/CT into the clinical standard for postablative staging in patients with DTC is recommended as well as utilization of SPECT/CT during the planning process, for dose calculation and treatment monitoring of SIRT therapies. Die diagnostische Genauigkeit dieser szintigraphischen Methode ist jedoch aufgrund limitierter raeumlicher Aufloesung begrenzt. Die Radioembolisation von mit {sup 90}Yttrium beladenen Mikrosphaeren (selektive interne Radiotherapie, SIRT) ermoeglicht eine wenig invasive Therapie primaerer und sekundaerer Lebertumoren. Zur Vermeidung von Nebenwirkungen der Mikrosphaeren durch einen Abstrom in Darm, Magen und Lunge muss vor Therapiebeginn eine Darstellung des durch die leberversorgenden Arterien versorgten Gebiets mittels {sup 99m}Tc-MAA ({sup 99m}Technetium-makroaggregiertes Albumin) und einer Szintigraphie erfolgen.     Purpose: In the course of radiotherapy oncological patients often experience considerable psychosocial distress. For its measurement however, no specific questionnaire is available. The stress index radiooncology (SIRO), which is based upon the results of extensive preliminary studies, will be made available as a screening-instrument to facilitate measurement of psychosocial distress of cancer patients, including radiotherapy-induced distress. The aim of this study is, to psychometrically evaluate the preliminary version of the questionnaire, to transfer it to the final version (SIRO) and to gain information about the psychosocial distress of radiooncological patients at the beginning of radiotherapy. Patients and Methods: 104 cancer patients (18 to 85 years) with different diagnoses have been included in the study (Table 1). The data have been assessed by means of the preliminary version of the new questionnaire SIRO, the HADS, EORTC QLQ-C30 and LS. With 25 patients semistructured clinical interviews have been conducted. [German] Hintergrund: Die strahlentherapeutische Behandlung von Tumorpatienten ist haeufig mit starken psychosozialen Belastungen verbunden, fuer deren Erfassung es bisher keinen spezifischen Fragebogen gibt. Mit dem stress index radioonkologie (SIRO), der auf den Analyseergebnissen umfangreicher Voruntersuchungen beruht, soll erstmals ein Screening-Instrument zur Verfuegung gestellt werden, mit dem die psychosozialen Belastungen von Tumorpatienten, einschliesslich der durch Strahlentherapie induzierten, erfasst werden koennen. Ziel dieser Studie ist, die Fragebogen-Vorform psychometrisch zu evaluieren und in die endgueltige Fragebogenversion (SIRO) zu ueberfuehren. Darueber hinaus sollen Informationen ueber das psychosoziale Belastungsprofil radioonkologischer Patienten zu Beginn einer Strahlentherapie gewonnen werden. Patienten und Methoden: 104 Tumorpatienten (18-85 Jahre) mit unterschiedlichen Diagnosen wurden in die Studie aufgenommen. Die Patientendaten wurden mit der SIRO-Vorform, der Hospital Anxiety and Depression Scale (HADS), der European Organization for Research and Treatment of Cancer Study Group Quality of Life Questionnaire (EORTC QLQ-C30) und der Lebenssituation (LS) erfasst. Mit 25 Patienten wurden halb standardisierte Interviews durchgefuehrt. Ergebnisse: Die Anforderungen an die Guetekriterien (Reliabilitaet, Validitaet) des SIRO wurden erfuellt bzw. uebertroffen. In der untersuchten Patientenstichprobe traten die hoechsten Belastungswerte in der Skala ''Psychophysische Belastungen'' auf, gefolgt von den Skalen ''Partnerschaftliche Probleme'', ''Strahlentherapeutische Belastungen'' und ''Informationsdefizite''. Entsprechend waren auf Itemebene die hoechsten Belastungen bei den Leistungseinschraenkungen und Aengsten zu beobachten. Bei den strahlentherapeutischen Items waren die Patienten am staerksten durch die Angst vor Nebenwirkungen der Strahlentherapie und die Tatsache, dass sie bestrahlt werden, belastet. Patienten mit palliativer Behandlungsoption waren staerker als solche mit kurativer belastet. Schlussfolgerungen: Die Fragebogen-Vorform hat sich als reliabel, valide und praktikabel erwiesen und kann unveraendert als neuer Selbstberichts-Fragebogen (SIRO) zur Erfassung der psychosozialen Belastung radioonkologischer Patienten uebernommen werden. Als Mengenindikator verwendeten wir die Publikationsrate und als Qualitaetsindikator den ''2012 Citations Index Impact Factor''. Die Arbeiten wurden ausserdem geordnet nach Studientyp, wissenschaftlichem Fokus, Tumorentitaet, Art der Zeitschrift sowie Erscheinungsjahr. Um die Unterschiede zwischen den Mittelwerten der untersuchten Gruppen zu bestimmen, wurde eine Einwegvarianzanalyse (ANOVA) durchgefuehrt. Von insgesamt 207 Fachzeitschriften wurde signifikant haeufiger in jenen der Chirurgie (n = 399; 41 %) und der Strahlentherapie (n = 273; 28 %; p < 0,01) publiziert, wobei der hoechste Impactfaktor in Fachjournalen der klinischen Onkologie erzielt wurde (p < 0,01). Die meisten Veroeffentlichungen bezogen sich auf retrospektive Kohortenstudien (n = 622; 64 %), gefolgt von Uebersichtsarbeiten (n = 204; 21 %; p < 0,001). Zu klinischen Ergebnissen nach spezifischer Tumortherapie erschienen im Vergleich zu anderen Themen die zahlreichsten Arbeiten (n = 661; 68 %, p < 0,001); hier wurde auch der hoechste mittlere Impactfaktor erreicht (p < 0,01). Am haeufigsten fanden sich Veroeffentlichungen ueber gastrointestinale Tumoren (n = 456; 47 %; p < 0,001); im Bereich gynaekologischer Tumoren und Mammakarzinome wurde dagegen der hoechste mittlere Impactfaktor erzielt (p < 0,01). Es war festzustellen, dass die wissenschaftliche Produktivitaet internationaler IORT-Expertengruppen bestaendig und nachhaltig ist.     Full Text Available Abstract Background Researchers are finding limitations of currently available disease-focused questionnaire tools for outcome studies in complementary and alternative medicine/integrative medicine (CAM/IM. Methods Three substudies investigated the new one-item visual analogue Arizona Integrative Outcomes Scale (AIOS, which assesses self-rated global sense of spiritual, social, mental, emotional, and physical well-being over the past 24 hours and the past month. The first study tested the scale's ability to discriminate unhealthy individuals (n = 50 from healthy individuals (n = 50 in a rehabilitation outpatient clinic sample. The second study examined the concurrent validity of the AIOS by comparing ratings of global well-being to degree of psychological distress as measured by the Brief Symptom Inventory (BSI in undergraduate college students (N = 458. The third study evaluated the relationships between the AIOS and positively- and negatively-valenced tools (Positive and Negative Affect Scale and the Positive States of Mind Scale in a different sample of undergraduate students (N = 62. Results Substudy (i Rehabilitation patients scored significantly lower than the healthy controls on both forms of the AIOS and a current global health rating. The AIOS 24-hours correlated moderately and significantly with global health (patients r = 0.50; controls r = 0.45.   16. Automatic interactive optimization for volumetric modulated arc therapy planning

    Intensity modulated radiotherapy treatment planning for sites with many different organs-at-risk (OAR) is complex and labor-intensive, making it hard to obtain consistent plan quality. With the aim of addressing this, we developed a program (automatic interactive optimizer, AIO) designed to automate the manual interactive process for the Eclipse treatment planning system. We describe AIO and present initial evaluation data. Our current institutional volumetric modulated arc therapy (RapidArc) planning approach for head and neck tumors places 3-4 adjustable OAR optimization objectives along the dose-volume histogram (DVH) curve that is displayed in the optimization window. AIO scans this window and uses color-coding to differentiate between the DVH-lines, allowing it to automatically adjust the location of the optimization objectives frequently and in a more consistent fashion. We compared RapidArc AIO plans (using 9 optimization objectives per OAR) with the clinical plans of 10 patients, and evaluated optimal AIO settings. AIO consistency was tested by replanning a single patient 5 times. Average V95&V107 of the boost planning target volume (PTV) and V95 of the elective PTV differed by ≤0.5%, while average elective PTV V107 improved by 1.5%. Averaged over all patients, AIO reduced mean doses to individual salivary structures by 0.9-1.6Gy and provided mean dose reductions of 5.6Gy and 3.9Gy to the composite swallowing structures and oral cavity, respectively. Re-running AIO five times, resulted in the aforementioned parameters differing by less than 3%.     This study addressed the effects of dietary Lactobacillus plantarum or/and N-acylated homoserine lactonase (AHL lactonase) on controlling Aeromonas  hydrophila infection in juvenile hybrid tilapia (Oreochromis niloticus♀ × O. aureus ♂). Fish were fed Lb. plantarum subsp. plantarum strain JCM1149 (10(8)  CFU/g feed) or/and AHL lactonase AIO6 (4 U/g) and were exposed to a chronic challenge of A. hydrophila NJ-1 (10(5)  cells/mL) for 14 days. Intestinal (foregut) alkaline phosphatase (IAP) activities were evaluated 1 day post challenge to reflect the resistance of fish against A. hydrophila infection. Parallel groups of fish with the same dietary assignments while unchallenged were also included to investigate the effect of dietary Lb. plantarum or/and AIO6 supplementation on gut health of tilapia. The results showed that IAP activity was significantly lower in fish fed with diets supplemented with Lb. plantarum JCM1149 or the combination of Lb. plantarum JCM1149 and AIO6, indicating enhanced resistance against A. hydrophila. Light microscopy and transmission electron microscopy images of foregut revealed damage caused by A. hydrophila NJ-1, but dietary Lb. plantarumJCM1149 or/and AIO6 significantly alleviated the damages. Compared to the fish immersed in A. hydrophila NJ-1, dietary Lb. plantarum JCM1149 or AIO6 could maintain the microvilli length in the foregut of tilapia. However, among the unchallenged groups of fish, the microvilli length in the foregut of tilapia fed AIO6 (singly or combination) and the microvilli density of tilapia fed AIO6 (singly) were significantly lower than those of the control, though the microvilli density in the combination treatment was significantly improved. Additionally, the dietary Lb. plantarum JCM1149 could down-regulate the expression of stress-related gene in the gut after the acute phase.     Antimonite [Sb(III)]-oxidizing bacteria can transform the toxic Sb(III) into the less toxic antimonate [Sb(V)]. Recently, the cytoplasmic Sb(III)-oxidase AnoA and the periplasmic arsenite [As(III)] oxidase AioAB were shown to responsible for bacterial Sb(III) oxidation, however, disruption of each gene only partially decreased Sb(III) oxidation efficiency. This study showed that in Agrobacterium tumefaciens GW4, Sb(III) induced cellular H2O2 content and H2O2 degradation gene katA. Gene knock-out/complementation of katA, anoA, aioA and anoA/aioA and Sb(III) oxidation and growth experiments showed that katA, anoA and aioA were essential for Sb(III) oxidation and resistance and katA was also essential for H2O2 resistance. Furthermore, linear correlations were observed between cellular H2O2 and Sb(V) content in vivo and chemical H2O2 and Sb(V) content in vitro (R2 = 0.93 and 0.94, respectively). These results indicate that besides the biotic factors, the cellular H2O2 induced by Sb(III) also catalyzes bacterial Sb(III) oxidation as an abiotic oxidant. The data reveal a novel mechanism that bacterial Sb(III) oxidation is associated with abiotic (cellular H2O2) and biotic (AnoA and AioAB) factors and Sb(III) oxidation process consumes cellular H2O2 which contributes to microbial detoxification of both Sb(III) and cellular H2O2.

    Full Text Available Lysosomale Speicherkrankheiten sind eine klinisch und biochemisch heterogene Gruppe von genetischen Erkrankungen, die ihren Ursprung in einer Dysfunktion von lysosomalen Stoffwechselvorgängen haben. Lysosomale Speicherkrankheiten sind dramatisch unterdiagnostiziert, obwohl für eine ständig wachsende Anzahl der Erkrankungen wirksame Therapien zur Verfügung stehen. Klinisch manifestieren sich lysosomale Speicherkrankheiten mit neurologischen Störungen, skelettalen Veränderungen, Hepatosplenomegalie und kardiovaskulären Ereignissen und stellen damit wichtige Differenzialdiagnosen häufiger internistischer Erkrankungen dar. In dieser Übersicht wird die Pathophysiologie lysosomaler Speicherkrankheiten dargelegt und anhand besonders relevanter Beispiele (Mb. Gaucher, Mb. Fabry, Mb.     The goal of this work was to compare the TL properties of various electrochemically produced alumina layers (E-AIO) in order to investigate the effect of the electrolyte and the Mg content on the alloys. It has been found that the TL sensitivity of oxidised layers is more influenced by the type of electrolyte, than by the composition of alloy. Hard oxide layer evolved in reduction electrolyte has rather different character compared to other alumina production investigated. The effect of reducing media seems to be very important during preparation of alumina layer. One of the advantages properties of E-AIO is, that it serve a promising method to increase the measuring range of TL method above 10 kGy as well. (author)

Por isso, necessitam de instrumentos que permitam agilidade para adaptar suas estratégias, processos e sistemas às necessidades de cada momento. A Arquitetura Empresarial (AE tem sido identificada por pesquisadores como a abordagem adequada para prover a agilidade organizacional, pois permite o conhecimento dos elementos organizacionais favorecendo planejar e realizar as mudanças necessárias. Entretanto, a AE ainda não é uma realidade para a maioria das organizações. Os motivos são a complexidade inerente, a falha das abordagens atuais para lidar com essa complexidade e o custo das ferramentas envolvidas. A AE é uma disciplina em desenvolvimento e requer princípios, teorias e instrumentos adequados à sua natureza e acessíveis a organizações de qualquer porte. Nesse contexto, este estudo identifica um novo referencial teórico: a AE como Arquitetura de Informação Organizacional (AIO. O estudo propõe, ainda, uma abordagem para a AIO, que se fundamenta nos princípios da colaboração organizacional e nos métodos ágeis. Seu principal objetivo é reduzir a complexidade e o custo da AIO. A solução proposta é avaliada em estudo de caso em uma instituição do sistema financeiro brasileiro.

Because of its unique mechanism of action it shows activity in relapsed indolent lymphomas which are resistant to alkylating agents, purine analogs, and rituximab. Bendamustine has a favorable toxicity profile causing no alopecia and only a moderate hematotoxicity and gastrointestinal toxicity. Combinations of bendamustine with mitoxantrone and rituximab and with rituximab alone have been shown to be highly active in relapsed/refractory indolent lymphomas and mantle cell lymphomas achieving long lasting complete remissions. Because of only moderate toxicity these combinations can be applied safely in elderly patients who can be treated in an outpatient setting.Keywords: bendamustine, relapsed-indolent, non-Hodgkin’s lymphoma

    To test the applicability of the Arbeitsgemeinschaft Gynäkologische Onkologie (AGO) and Memorial Sloan Kettering (MSK) criteria in predicting complete cytoreduction (CC) in patients undergoing secondary cytoreductive surgery (SCS) for recurrent ovarian cancer (ROC). Data of consecutive patients undergoing SCS were reviewed. The Arbeitsgemeinschaft Gynäkologische Onkologie OVARian cancer study group (AGO-OVAR) and MSK criteria were retrospectively applied. Nomograms, based on AGO criteria, MSK criteria and both AGO and MSK criteria were built in order to assess the probability to achieve CC at SCS. Overall, 194 patients met the inclusion criteria. CC was achieved in 161 (82.9%) patients. According to the AGO-OVAR criteria, we observed that CC was achieved in 87.0% of patients with positive AGO score. However, 45 out of 71 (63.4%) patients who did not fulfilled the AGO score had CC. Similarly, CC was achieved in 87.1%, 61.9% and 66.7% of patients for whom SCS was recommended, had to be considered and was not recommended, respectively. In order to evaluate the predictive value of the AGO-OVAR and MSK criteria we built 2 separate nomograms (c-index: 0.5900 and 0.5989, respectively) to test the probability to achieve CC at SCS. Additionally, we built a nomogram using both the aforementioned criteria (c-index: 0.5857). The AGO and MSK criteria help identifying patients deserving SCS. However, these criteria might be strict, thus prohibiting a beneficial treatment in patients who do not met these criteria. Further studies are needed to clarify factors predicting CC at SCS.   10. Determination of physiological, taxonomic, and molecular characteristics of a cultivable arsenic-resistant bacterial community.

    A collection of 219 bacterial arsenic-resistant isolates was constituted from neutral arsenic mine drainage sediments. Isolates were grown aerobically or anaerobically during 21 days on solid DR2A medium using agar or gelan gum as gelling agent, with 7 mM As(III) or 20 mM As(V) as selective pressure. Interestingly, the sum of the different incubation conditions used (arsenic form, gelling agent, oxygen pressure) results in an overall increase of the isolate diversity. Isolated strains mainly belonged to Proteobacteria (63%), Actinobacteria (25%), and Bacteroidetes (10%). The most representative genera were Pseudomonas (20%), Acinetobacter (8%), and Serratia (15%) among the Proteobacteria; Rhodococcus (13%) and Microbacterium (5%) among Actinobacteria; and Flavobacterium (13%) among the Bacteroidetes. Isolates were screened for the presence of arsenic-related genes (arsB, ACR3(1), ACR3(2), aioA, arsM, and arrA). In this way, 106 ACR3(1)-, 74 arsB-, 22 aioA-, 14 ACR3(2)-, and one arsM-positive PCR products were obtained and sequenced. Analysis of isolate sensitivity toward metalloids (arsenite, arsenate, and antimonite) revealed correlations between taxonomy, sensitivity, and genotype. Antimonite sensitivity correlated with the presence of ACR3(1) mainly present in Bacteroidetes and Actinobacteria, and arsenite or antimonite resistance correlated with arsB gene presence. The presence of either aioA gene or several different arsenite carrier genes did not ensure a high level of arsenic resistance in the tested conditions.

  11. Coordinated airborne and satellite measurements of equatorial plasma depletions

    A series of experiments was conducted in December 1979 to investigate the structure of plasma depletions in the low latitude, nightime ionosphere. The measurements included all sky imaging photometer (ASIP), ionosonde and amplitude scintillation observations from the AFGL Airborne Ionospheric Observatory (AIO), and in situ ion density measurements from the Atmosphere Explorer (AE-E) Bennett Ion Mass Spectrometer (BIMS). The AIO performed two flights along the Ascension Island (-18 0 MLAT) magnetic meridian: one in the southern hemisphere and one near the Ascension conjugate point in the northern hemisphere. During these flights, measurements from the AE-E satellite at 434 km altitude are compared with simultaneous remote ionospheric measurements from the AIO. Density biteouts of approximately one order of magnitude in the dominant ion O + , were mapped to lower altitudes along magnetic field lines for comparison with 6300-A and 7774-A O I airglow depletions. Because of the different airglow production mechanisms (dissociative recombination of O +2 for 6300 A and radiative recombination of O + for 7774 A) the 6300-A depletions reflect plasma depletions near the bottomside of the F layer, while those at 7774 A are located near the peak of the layer. The O + biteouts map directly into the 7774-A airglow depletions in the same hemisphere and also when traced into the opposite hemisphere, which indicates magnetic flux tube alignment over north-south distances of approx.2220 km. The 6300-A (bottomside) depletions are wider in longitude than the 7774-A (F-peak) depletions near the equatorward edge of the Appleton anomaly. This difference in topside and bottomside structure is used to infer large-scale structure near the anomaly and to relate this to structure, commonly observed near the magnetic equator by the ALTAIR radar

  12. Microbiological oxidation of antimony(III) with oxygen or nitrate by bacteria isolated from contaminated mine sediments

    Bacterial oxidation of arsenite [As(III)] is a well-studied and important biogeochemical pathway that directly influences the mobility and toxicity of arsenic in the environment. In contrast, little is known about microbiological oxidation of the chemically similar anion antimonite [Sb(III)]. In this study, two bacterial strains, designated IDSBO-1 and IDSBO-4, which grow on tartrate compounds and oxidize Sb(III) using either oxygen or nitrate, respectively, as a terminal electron acceptor, were isolated from contaminated mine sediments. Both isolates belonged to the Comamonadaceae family and were 99% similar to previously described species. We identify these novel strains as Hydrogenophagataeniospiralis strain IDSBO-1 and Variovorax paradoxus strain IDSBO-4. Both strains possess a gene with homology to the aioA gene, which encodes an As(III)-oxidase, and both oxidize As(III) aerobically, but only IDSBO-4 oxidized Sb(III) in the presence of air, while strain IDSBO-1 could achieve this via nitrate respiration. Our results suggest that expression of aioA is not induced by Sb(III) but may be involved in Sb(III) oxidation along with an Sb(III)-specific pathway. Phylogenetic analysis of proteins encoded by the aioA genes revealed a close sequence similarity (90%) among the two isolates and other known As(III)-oxidizing bacteria, particularly Acidovorax sp. strain NO1. Both isolates were capable of chemolithoautotrophic growth using As(III) as a primary electron donor, and strain IDSBO-4 exhibited incorporation of radiolabeled [14C]bicarbonate while oxidizing Sb(III) from Sb(III)-tartrate, suggesting possible Sb(III)-dependent autotrophy. Enrichment cultures produced the Sb(V) oxide mineral mopungite and lesser amounts of Sb(III)-bearing senarmontite as precipitates.

    Microbial arsenite (AsIII) oxidation forms a critical piece of the arsenic cycle in nature, though our understanding of how and why microorganisms oxidize AsIII remains rudimentary. Our model organism Agrobacterium tumefaciens 5A contains two distinct ars operons (ars1 and ars2) that are similar in their coding region content. The ars1 operon is located nearby the aio operon that is essential for AsIII oxidation. The AsIII/H(+) anti-porters encoded by acr3-1 and acr3-2 are required for maximal AsIII and antimonite (SbIII) resistance, but acr3-1 (negatively regulated by ArsR-1) appears more active in this regard and also required for AsIII oxidation and expression of aioBA. A malate-phosphate anti-porter DctA is regulated by RpoN and AsIII, and is required for normal growth with malate as a sole carbon source. Qualitatively, a ΔdctA mutant was normal for AsIII oxidation and AsIII/SbIII resistance at metalloid concentrations inhibitory to the Δacr3-1 mutant; however, aioBA induction kinetics was significantly phase-shift delayed. Acr3 involvement in AsIII/SbIII resistance is reasonably well understood, but the role of Acr3 and DctA anti-porters in AsIII oxidation and its regulation is unexpected, and suggests that controlled AsIII trafficking across the cytoplasmic membrane is important to a process understood to occur in the periplasm. © 2014 Society for Applied Microbiology and John Wiley & Sons Ltd.

    Microbes play an important role in arsenic transformation and cycling in the environment. Microbial arsenic oxidation and reduction were demonstrated in the growth media of arsenic hyperaccumulator Pteris vittata L. All arsenite (AsIII) at 0.1 mM in the media was oxidized after 48 h incubation. Oxidation was largely inhibited by antibiotics, indicating that bacteria played a dominant role. To identify AsIII oxidizing bacteria, degenerate primers were used to amplify ∼500 bp of the AsIII oxidase gene aioA (aroA) using DNA extracted from the media. One aioA (aroA)-like sequence (MG-1, tentatively identified as Acinetobacter sp.) was amplified, exhibiting 82% and 91% identity in terms of gene and deduced protein sequence to those from Acinetobacter sp. 33. In addition, four bacterial strains with different arsenic tolerance were isolated and identified as Comamonas sp.C-1, Flavobacterium sp. C-2, Staphylococcus sp. C-3, and Pseudomonas sp. C-4 using carbon utilization, fatty acid profiles, and/or sequencing 16s rRNA gene. These isolates exhibited dual capacity for both AsV reduction and AsIII oxidation under ambient conditions. Arsenic-resistant bacteria with strong AsIII oxidizing ability may have potential to improve bioremediation of AsIII-contaminated water using P. vittata and/or other biochemical strategies.

    This study aimed to examine the effects of kinesiology taping (KT) and different TRX suspension workouts on the amplitude of electromyographic (EMG) activity in the core muscles among people with chronic low back pain (LBP). Each participant (total n = 21) was exposed to two KT conditions: no taping and taping, while performing four TRX suspension exercises: (1) hamstring curl, (2) hip abduction in plank, (3) chest press, and (4) 45-degree row. Right transversus abdominis/internal oblique (TrAIO), rectus abdominis (RA), external oblique (EO), and superficial lumbar multifidus (LMF) activity was recorded with surface EMG and expressed as a percentage of the EMG amplitude recorded during a maximal voluntary isometric contraction of the respective muscles. Hip abduction in plank increased TrAIO, RA, and LMF EMG amplitude compared with other TRX positions (P hamstring curl was effective in inducing a high EMG amplitude of LMF (P 0.05). Hip abduction in plank most effectively activated abdominal muscles, whereas the hamstring curl most effectively activated the paraspinal muscles. Applying KT conferred no immediate benefits in improving the core muscle activation during TRX training in adults with chronic LBP.

    Full Text Available Using vowel polygons, exactly their parameters, is chosen as the criterion for achievement of differences between normal state of speaker and relevant speech under real psychological stress. All results were experimentally obtained by created software for vowel polygon analysis applied on ExamStress database. Selected 6 methods based on cross-correlation of different features were classified by the coefficient of variation and for each individual vowel polygon, the efficiency coefficient marking the most significant and uniform differences between stressed and normal speech were calculated. As the best method for observing generated differences resulted method considered mean of cross correlation values received for difference area value with vector length and angle parameter couples. Generally, best results for stress detection are achieved by vowel triangles created by /i/-/o/-/u/ and /a/-/i/-/o/ vowel triangles in formant planes containing the fifth formant F5 combined with other formants.

    Full Text Available OBJETIVOS: este trabalho de pesquisa teve o intuito de realizar uma avaliação clínica e polissonográfica do efeito de um aparelho intraoral (AIO para tratamento da Síndrome da Apneia Obstrutiva do Sono (SAOS, desenvolvido e testado por duas universidades federais brasileiras. MÉTODOS: a amostra constou de 50 pacientes (idades entre 18 e 65 anos, sendo 33 homens e 17 mulheres com diagnóstico polissonográfico inicial de SAOS de grau leve e moderado. Todos os pacientes submeteram-se a uma nova avaliação polissonográfica de noite inteira (em uso do AIO aproximadamente 6 meses após a primeira avaliação. Baseado na diminuição dos eventos respiratórios obstrutivos, obtida com o uso do AIO, os pacientes foram então divididos em bons respondedores (redução de 50% ou mais no índice de apneia e hipopneia (IAH, permanecendo abaixo de 10 eventos/hora e maus respondedores (IAH permanecendo maior ou igual a 10 eventos/hora. RESULTADOS E CONCLUSÕES: em 54% da amostra o IAH diminuiu para menos de cinco eventos/hora com o uso do AIO; em 38% a redução do IAH foi maior do que 50%, mas permaneceu acima de cinco eventos/hora; e em 6% da amostra o IAH reduziu menos que 50%. Os bons respondedores corresponderam a 86% da amostra estudada, enquanto os maus respondedores a 14%. Houve melhora significativa na escala de sonolência, no IAH, nos microdespertares e na saturação mínima de oxihemoglobina com a terapia utilizada.     The fusion of cross-sectional images - especially in oncology - appears to be a very helpful tool to improve the diagnostic and therapeutic accuracy. Though many advantages exist, image fusion is applied routinely only in a few hospitals. To introduce image fusion as a common procedure, technical and logistical conditions have to be fulfilled which are related to long term archiving of digital data, data transfer and improvement of the available software in terms of usefulness and documentation. The accuracy of coregistration and the quality of image fusion has to be validated by further controlled studies. (orig.) [German] Zur Erhoehung der diagnostischen und therapeutischen Sicherheit ist die Fusion von Schnittbildern verschiedener tomographischer Verfahren insbesondere in der Onkologie sehr hilfreich. Trotz bestehender Vorteile hat die Bildfusion bisher nur in einzelnen Zentren Einzug in die nuklearmedizinische und radiologische Routinediagnostik gefunden. Um die Bildfusion allgemein einsetzen zu koennen, sind bestimmte technische und logistische Voraussetzungen notwendig.   4. Is a diagnostic CT of the brain indicated in patients with choroidal metastases before radiotherapy?

    Background and purpose: there is no evidence in the literature about the incidence of synchronous brain metastases in patients with choroidal metastases. This is of major importance, because the radiation fields of choroidal metastases and, later on, brain metastases, if treated consecutively, are partly overlapping, thus potentially increasing the rate of late side effects such as brain necrosis. The goal of this study was to determine the frequency of synchronous brain metastases. Patients and methods: 50 patients with choroidal metastases were enrolled into a study of the ''Arbeitsgemeinschaft Radiologische Onkologie'' of the German Cancer Society (ARO 95-08) with standardized 40 Gy radiotherapy, 2 Gy single dose. All patients were staged before treatment with a computed tomography of the brain (CCT). No patient showed clinical signs of brain metastases. Results: 13 out of 50 patients (26%) had brain metastases in the CCT leading to radiotherapy of the brain and choroidal metastases in one volume. Conclusion: a CCT is indicated at the diagnosis of choroidal metastases for screening of synchronous brain metastases. The incidence is about 25%, and the diagnosis of brain metastases results in a different target volume: the whole brain including the posterior parts of the eyes compared to the posterior parts of the eyes alone. Therefore, the risk of late side effects could be reduced compared with an additional later radiotherapy of the whole brain with partly overlapping fields. (orig.)

  5. [Efficacy of a ketogenic diet in urological cancers patients : A systematic review].

    Beside the classical anticancer treatment tumor patients try to find proactive alternative therapies to fight their disease. Lifestyle changes such as introducing a ketogenic diet is one of the most popular among them. The German Association of Urological Oncology (AUO, Arbeitsgemeinschaft Urologische Onkologie) presents a systematic review investigating the evidence of ketogenic diet in cancer patients. A systematic literature research was conducted in the databases Medline, Livivo, and the Cochrane Library. Only clinical studies of tumor patients receiving chemotherapy while on a ketogenic diet were included. The assessment of the results was performed according to the predefined primary endpoints overall survival and progression-free survival and secondary endpoints quality of life and reduction of adverse effects induced by cytostatics. Nine studies met the inclusion criteria: eight prospective and one retrospective study case series respectively cohort-studies, with a total of 107 patients. Currently there is no evidence of a therapeutic effect of a ketogenic diet in patients with malignant tumors regarding the clinical outcome or quality of life. Based on the current data, a ketogenic diet can not be recommended to cancer patients because prospective, randomized trials are missing.

    Full Text Available Abstract Background Preterm infants need high amounts of calcium and phosphorus for bone mineralization, which is difficult to obtain with parenteral feeding due to the low solubility of these salts. The objective of this study was to evaluate the physicochemical compatibility of high concentrations of calcium associated with organic phosphate and its influence on the stability of AIO admixtures for neonatal use. Methods Three TPN admixture formulas were prepared in multilayered bags. The calcium content of the admixtures was adjusted to 0, 46.5 or 93 mg/100 ml in the presence of a fixed organic phosphate concentration as well as lipids, amino acids, inorganic salts, glucose, vitamins and oligoelements at pH 5.5. Each admixture was stored at 4°C, 25°C or 37°C and evaluated over a period of 7 days. The physicochemical stability parameters evaluated were visual aspect, pH, sterility, osmolality, peroxide formation, precipitation, and the size of lipid globules. Results Color alterations occurred from the first day on, and reversible lipid film formation from the third day of study for the admixtures stored at 25°C and 37°C. According to the parameters evaluated, the admixtures were stable at 4°C; and none of them presented precipitated particles due to calcium/phosphate incompatibility or lipid globules larger than 5 μm, which is the main parameter currently used to evaluate lipid emulsion stability. The admixtures maintained low peroxide levels and osmolarity was appropriate for parenteral administration.     Full Text Available The presence of the arsenic oxidation, reduction, and extrusion genes arsC, arrA, aioA, and acr3 was explored in a range of natural environments in northern Chile, with arsenic concentrations spanning six orders of magnitude. A combination of primers from the literature and newly designed primers were used to explore the presence of the arsC gene, coding for the reduction of As (V to As (III in one of the most common detoxification mechanisms. Enterobacterial related arsC genes appeared only in the environments with the lowest As concentration, while Firmicutes-like genes were present throughout the range of As concentrations. The arrA gene, involved in anaerobic respiration using As (V as electron acceptor, was found in all the systems studied. The As (III oxidation gene aioA and the As (III transport gene acr3 were tracked with two primer sets each and they were also found to be spread through the As concentration gradient. Sediment samples had a higher number of arsenic related genes than water samples. Considering the results of the bacterial community composition available for these samples, the higher microbial phylogenetic diversity of microbes inhabiting the sediments may explain the increased number of genetic resources found to cope with arsenic. Overall, the environmental distribution of arsenic related genes suggests that the occurrence of different ArsC families provides different degrees of protection against arsenic as previously described in laboratory strains, and that the glutaredoxin (Grx-linked arsenate reductases related to Enterobacteria do not confer enough arsenic resistance to live above certain levels of As concentrations.

Two major forms of As present in the environment As (III and As (V as compared to As (V, As (III is much more toxic. Our aim is to remediate As (III contaminated soil by using As (III resistant microbe, having the high transforming ability for As (III to As (V. An As (III oxidizing bacterium Bacillus sp. XS2, isolated and selected from gold mine tailing which have shown high resistance up to 6400 mg L−1 and efficiently transformed up to 4000 mg L−1 in sucrose low phosphate medium (SLP, higher than any of the previous reported Bacillus sp.. In soil, we found that XS2 successfully transformed up to 81% of soluble exchangeable fraction within 10 d in contaminated soil (with 500 mg kg−1, which makes it potential microbe for the removal of contaminated site with arsenic in the environment. Further XS2 was characterized for their molecular basis of resistance and we establish that XS2 having arsenic oxidase enzyme activity which is accountable for the detoxification of arsenic at high concentration and provide resistance to the bacterium. Gene encoding arsenic oxidase (aioA-gene was also amplified from this bacterium using polymerase chain reaction (PCR with degenerate primer. The aioA-gene is specific for the arsenite-oxidizing bacteria. The deduced amino acid sequence had shown 42% similarity with pseudomonas sp. arsenic oxidase. Further, the strain was characterized by 16s rRNA gene sequence analysis and shown maximum similarity with Bacillus sp.

    The median age of patients with Ewing sarcoma (EwS) at diagnosis is around 14-15 years. Older age is associated with a worse outcome. The correlation of age at diagnosis on sites of disease has not been fully described. The goal of this study was to evaluate the differences in sites of primary tumor and metastatic tumor involvement according to age groups. EwS data from the Gesellschaft für Pädiatrische Onkologie und Hämatology (GPOH) database of the Cooperative Ewing Sarcoma Study (CESS) 81/86 and the European Intergroup Cooperative Ewing's Sarcoma Study EICESS 92 and the EUROpean Ewing tumor Working Initiative of National Groups-99-Protocol (EURO-E.W.I.N.G.-99) study were analyzed. Patient and tumor characteristics were evaluated statistically using chi square tests. The study population included 2,635 patients with bone EwS. Sites of primary and metastatic tumors differed according to the age groups of young children (0-9 years), early adolescence (10-14 years), late adolescence (15-19 years), young adults (20-24 years), and adults (more than 24 years). Young children demonstrated the most striking differences in site of disease with a lower proportion of pelvic primary and axial tumors. They presented less often with metastatic disease at diagnosis. Site of primary and metastatic tumor involvement in EwS differs according to patient age.     Positron emission tomography with FDG (FDG-PET) is a new technique, which displays the cellular metabolic activity. Since tumors exhibit an increased metabolic activity when compared to normal tissue, this imaging modality has a particularly high importance. FDG-PET is not only useful for localizing and staging of malignant tumors, but also to evaluate therapy response. In this context, PET is superior to morphologically orientated modalities, because therapeutically induced changes in glucose metabolism precede morphologic alterations. Numerous studies indicate, that PET will play an important role in radiooncology concerning therapy planning and monitoring the effects of therapy during and after treatment. Further clinical studies are necessary to evaluate the information provided by FDG-PET more precisely.     The increasing use of immunotherapy in oncology increases the need for radiologic evaluation of frequent and severe immune-related adverse events. Determination of the incidence and manifestation of radiologic and nuclear medicine findings of immune-related adverse events. Literature review of clinical and imaging findings of immune-related adverse events induced by the immune checkpoint inhibitors ipilimumab, nivolumab and pembrolizumab. Findings are illustrated with pictorial examples and contrasted to other relevant differential diagnoses.     The growing popularity and acceptance of integrative medicine is evident both among patients and among the oncologists treating them. As little data are available regarding health-care professionals' knowledge, attitudes, and practices relating to the topic, a nationwide online survey was designed. Over a period of 11 weeks (from July 15 to September 30, 2014) a self-administered, 17-item online survey was sent to all 676 members of the Research Group on Gynecological Oncology (Arbeitsgemeinschaft Gynäkologische Onkologie) in the German Cancer Society. The questionnaire items addressed the use of integrative therapy methods, fields of indications for them, advice services provided, level of specific qualifications, and other topics. Of the 104 respondents (15.4%) using integrative medicine, 93% reported that integrative therapy was offered to breast cancer patients. The second most frequent type of tumor in connection with which integrative therapy methods were recommended was ovarian cancer, at 80% of the participants using integrative medicine. Exercise, nutritional therapy, dietary supplements, herbal medicines, and acupuncture were the methods the patients were most commonly advised to use. There is considerable interest in integrative medicine among gynecological oncologists, but integrative therapy approaches are at present poorly implemented in routine clinical work. Furthermore there is a lack of specific training. Whether future efforts should focus on extending counseling services on integrative medicine approaches in gynecologic oncology or not, have to be discussed. Evidence-based training on integrative medicine should be implemented in order to safely guide patients in their wish to do something by themselves.

    Full Text Available Este trabalho avalia a utilização do beam search filtrado (BSF combinado com um algoritmo de inserção de ociosidade (AIO. O caso estudado é baseado em uma única máquina, com datas de entrega múltiplas e com penalidades distintas de adiantamento e de atraso para cada ordem. O objetivo a ser alcançado é a minimização do custo total. Para isso, o BSF é utilizado para gerar as seqüências, e o AIO, para definir os programas. Assume-se que a dificuldade de solução do problema é dependente de dois parâmetros: fator de atraso médio e amplitude relativa das datas de entrega. Testes empíricos comparativos são realizados por meio de simulação computacional, no qual se mede o tempo de solução e o valor alcançado pela função-objetivo. Os resultados indicam que de uma forma geral, o procedimento proposto propicia uma diminuição no custo total e, além disso, que para a escolha de um procedimento apropriado, o ideal é se conhecer o valor dos parâmetros.This work evaluates the use of a filtered beam search (FBS combined with an ociosity insertion algorithm (OIA. The case studied is based on a single machine, with multiple due dates, and distinct earliness and tardiness penalties for each job. The objective function is to minimize total cost. For this, the FBS is used to construct sequences, and the OIA is used to obtain schedules. It is assumed that the degree of difficulty of solving the problem is dependent on two parameters: average tardiness factor and relative range of due dates. Comparative empirical tests were conducted with computational simulation, where computational solution time and the objective function value were evaluated. The results indicate that in general, the proposed procedure favours a decrease in total cost and, in addition, that before choosing an appropriate procedure, the ideal situation is to know the value of the parameters.

    This study explores the unique role of Ga in amorphous (a-) In[BOND]Ga[BOND]O oxide semiconductors through combined theory and experiment. It reveals substitutional effects that have not previously been attributed to Ga, and that are investigated by examining how Ga influences structure–property relationships in a series of pulsed laser deposited a-In[BOND]Ga[BOND]O thin films. Element-specific structural studies (X-ray absorption and anomalous scattering) show good agreement with the results of ab initio molecular dynamics simulations. This structural knowledge is used to understand the results of air-annealing and Hall effect electrical measurements. The crystallization temperature of a-IO is shown to increase by as much as 325 °C on substituting Ga for In. This increased thermal stability is understood on the basis of the large changes in local structure that Ga undergoes, as compared to In, during crystallization. Hall measurements reveal an initial sharp drop in both carrier concentration and mobility with increasing Ga incorporation, which moderates at >20 at% Ga content. This decline in both the carrier concentration and mobility with increasing Ga is attributed to dilution of the charge-carrying In[BOND]O matrix and to increased structural disorder. The latter effect saturates at high at% Ga.

    Full Text Available In this study, the raw wastewater (RW, and effluents from the acidogenic phase (AP and methanogenic phase (MP in a swine wastewater treatment plant were collected to investigate the occurrence and transformation of arsenic (As, as well as the abundance of As metabolism genes during the anaerobic digestion (AD process. The results showed that total concentrations of As generally decreased by 33–71% after AD. Further analysis showed that the As species of the dissolved fractions were present mainly as dimethylarsinic acid (DMA, with arsenite (As(III and arsenate (As(V as the minor species. Moreover, real-time PCR (qPCR results showed that As metabolism genes (arsC, arsenate reduction gene; aioA, arsenite oxidation gene and arsM, arsenite methylation gene were highly abundant, with arsM being predominant among the metabolism genes. This study provides reliable evidence on As biotransformation in swine wastewater treatment process, suggesting that AD could be a valuable treatment to mitigate the risk of As in wastewater.

    As an analogue of phosphorus, arsenic (As) has a biogeochemical cycle coupled closely with other key elements on the Earth, such as iron, sulfate and phosphate. It has been documented that microbial genes associated with As biotransformation are widely present in As-rich environments. Nonetheless, their presence in natural environment with low As levels remains unclear. To address this issue, we investigated the abundance levels and diversities of aioA, arrA, arsC and arsM genes in estuarine sediments at low As levels across Southeastern China to uncover biogeographic patterns at a large spatial scale. Unexpectedly, genes involved in As biotransformation were characterized by high abundance and diversity. The functional microbial communities showed a significant decrease in similarity along the geographic distance, with higher turnover rates than taxonomic microbial communities based on the similarities of 16S rRNA genes. Further investigation with niche-based models showed that deterministic processes played primary roles in shaping both functional and taxonomic microbial communities. Temperature, pH, total nitrogen concentration, carbon/nitrogen ratio and ferric iron concentration rather than As content in these sediments were significantly linked to functional microbial communities, while sediment temperature and pH were linked to taxonomic microbial communities. The oxygen flow rates have been varied in the range 0.00-3.44 sccm during sputtering. The X-ray diffraction data on these indium-doped silver oxide films show polycrystalline nature. With increasing oxygen flow rate, the carrier concentration, the Hall mobility and the electron mean free path decrease. These films show a very low positive temperature coefficient of resistivity {approx}3.40x10{sup -8} ohm-cm/K. The work function values for these films (measured by Kelvin probe technique) are in the range 4.81-5.07 eV. The high electrical resistivity indicate that the films are in the island state (size effects). Calculations of the partial ionic charge (by Sanderson's theory) show that indium doping in silver oxide thin films enhance the ionicity.

  1. Identification of Antimony- and Arsenic-Oxidizing Bacteria Associated with Antimony Mine Tailing

    Antimony (Sb) is a naturally occurring toxic element commonly associated with arsenic (As) in the environment and both elements have similar chemistry and toxicity. Increasing numbers of studies have focused on microbial As transformations, while microbial Sb interactions are still not well understood. To gain insight into microbial roles in the geochemical cycling of Sb and As, soils from Sb mine tailing were examined for the presence of Sb- and As-oxidizing bacteria. After aerobic enrichment culturing with AsIII (10 mM) or SbIII (100 μM), pure cultures of Pseudomonas- and Stenotrophomonas-related isolates with SbIII oxidation activities and a Sinorhizobium-related isolate capable of AsIII oxidation were obtained. The AsIII-oxidizing Sinorhizobium isolate possessed the aerobic arsenite oxidase gene (aioA), the expression of which was induced in the presence of AsIII or SbIII. However, no SbIII oxidation activity was detected from the Sinorhizobium-related isolate, suggesting the involvement of different mechanisms for Sb and As oxidation. These results demonstrate that indigenous microorganisms associated with Sb mine soils are capable of Sb and As oxidation, and potentially contribute to the speciation and mobility of Sb and As in situ.   2. Physicochemical, Mineralogical and Microscopic Evaluation of Sustainable Bricks Manufactured with Construction Wastes

    Full Text Available At an international level, enormous volumes of construction and demolition wastes are generated: 170 million tons/year in the USA, 500 million tons/year in the European Union (EU and 12 million tons/year in Mexico. Alternative uses for these heterogeneous materials, such as the manufacture of sustainable bricks, are potential solutions to this growing environmental issue. Based on previous studies, and in compliance with Mexican standards, four different types of secondary materials were utilized in the composition of a sustainable brick matrix. Temperature and solar radiation used for drying purposes were determined, as well as weight loss, resistance and initial maximum absorption. In order to characterize the resulting matrix, observations were made with a scanning electron microscope, and the chemical composition of the samples was determined by detecting basic compounds using mapping through SEM-EDS microanalysis, connected to the SEM unit. Finally, thermogravimetric analyses were performed to correlate mechanical and chemical behavior, and resistance to high temperatures of the mixtures. The results obtained showed that all-in-one (AiO is the most appropriate material for brick manufacturing, Opuntia ficus-indica mucilage improves physical properties, such as increased compressive strength and reduced water absorption, while wood residues, clay minerals and illite enhance mechanical properties.

    A two-step method was developed to extract microplastics from sediments. First, 1 kg sediments was pre-extracted using the air-induced overflow (AIO) method, based on fluidisation in a sodium chloride (NaCl) solution. The original sediment mass was reduced by up to 80%. As a consequence, it was possible to reduce the volume of sodium iodide (NaI) solution used for the subsequent flotation step. Recoveries of the whole procedure for polyethylene, polypropylene (PP), polyvinyl chloride (PVC), polyethylene terephthalate (PET), polystyrene and polyurethane with sizes of approximately 1 mm were between 91 and 99%. After being stored for one week in a 35% H 2 O 2 solution, 92% of selected biogenic material had dissolved completely or had lost its colour, whereas the tested polymers were resistant. Microplastics were extracted from three sediment samples collected from the North Sea island Norderney.   4. Bacteria diversity and arsenic mobilization in rock biofilm from an ancient gold and arsenic mine.

    In this paper we characterize the biofilm community from an ancient Złoty Stok gold and arsenic mine. Bacterial diversity was examined using a culture-independent technique based on 16S rRNA gene amplification, cloning and sequencing. We show that unexpectedly the microbial diversity of this community was extremely high (more than 190 OTUs detected), with the most numerous members from Rhizobiales (α-Proteobacteria). Although the level of rock biofilm diversity was similar to the microbial mat community we have previously characterized in the same adit, its taxonomic composition was completely different. Detailed analysis of functional arrA and aioA genes, chemical properties of siderophores found in pore water as well as the biofilm chemical composition suggest that the biofilm community contributes to arsenic pollution of surrounding water in a biogeochemical cycle similar to the one observed in bacterial mats.   5. Toward Millions of File System IOPS on Low-Cost, Commodity Hardware.

    We describe a storage system that removes I/O bottlenecks to achieve more than one million IOPS based on a user-space file abstraction for arrays of commodity SSDs. The file abstraction refactors I/O scheduling and placement for extreme parallelism and non-uniform memory and I/O. The system includes a set-associative, parallel page cache in the user space. We redesign page caching to eliminate CPU overhead and lock-contention in non-uniform memory architecture machines. We evaluate our design on a 32 core NUMA machine with four, eight-core processors. Experiments show that our design delivers 1.23 million 512-byte read IOPS. The page cache realizes the scalable IOPS of Linux asynchronous I/O (AIO) and increases user-perceived I/O performance linearly with cache hit rates. The parallel, set-associative cache matches the cache hit rates of the global Linux page cache under real workloads.

    Downstaging after neoadjuvant treatment is increasingly used as a prognostic factor and surrogate endpoint in clinical trials. However, in recent trials of neoadjuvant 5-fluorouracil-based chemoradiotherapy for rectal cancer, downstaging did not translate into a benefit with regard to either disease-free survival (DFS) or overall survival. By analyzing the 10-year outcome data of the German CAO/ARO/AIO-94 phase 3 trial, the authors demonstrated that significantly fewer patients had poor prognostic features (eg, ypT3-4, ypN1-2) after preoperative 5-fluorouracil-based chemoradiotherapy. Nevertheless, these patients with International Union for Cancer Control stage II disease were found to be at a higher risk of developing distant metastases and had poorer DFS compared with patients with corresponding TNM tumor (sub)groups in the postoperative treatment arm, whereas patients with International Union for Cancer Control stage III disease demonstrated a nonsignificant trend toward a worse outcome after preoperative treatment. Overall, DFS remained identical in both treatment arms. Thus, "downstage migration" after neoadjuvant treatment resembles the reverse of the Will Rogers phenomenon and therefore may not be a reliable endpoint for long-term outcomes. © 2015 American Cancer Society.

  7. Microporous Silico-Alumino Phosphate (SAPO) Compound For Uranium Sorption

    The zeotype novel materials I.e. SAPO structures have been prepared to have particular property such as sorption or cations exchanger which can accommodate the uranyl ions. The SAPO's model was built from certain mole ratio of SiO 2 +AIO 2 +PO 2 using various template compounds of tetra(alkyl)s aminium hydroxide viz. Tetra-methyl, tetra-ethyl and tetra-propyl aminium. Those formed materials have been analyzed using x-ray diffraction spectrometer , then the data have been complied using the supporting software videozeo package Ver.3.1. The result have shown that crystalline structures of microporous SAPO materials formed are SAPO-20, SAPO-34, and SAPO-40. The exchanger capacities of some zeolites, SAPOs and ASP(1:3) have been studied and their selectivity to uranyl ions in acid solution (ph<3.5) has been determined by static isotherm exchanger process. The result have revealed various selectivity factors and occurrence of absorption mechanism in SAPO-n materials. This promises that uranium separation could be more effective using SAPO-n than that using other zeolites . The chloride ions have slightly decreased the ion-exchange capacity, while that of fluoride ions increased the ion-exchange capacity of ASP(1:3) for the uranyl ions

  8. Preparation of micro/nano-structure superhydrophobic film on aluminum plates using galvanic corrosion method.

    A simple and novel approach has been developed to obtain a microporous film with compound nanoparticles on the surface of aluminum alloy substrate using the galvanic corrosion method. The wettability of the surface changes from hydrophilicity to superhydrophobicity after chemical modification with stearic acid (SA). The water contact angle (WCA) and sliding angle (WSA) of superhydrophobic aluminum alloy surface (SAAS) are 154 degrees and 9 degrees, respectively. The roughness of the aluminum substrate increases after the oxidation reaction. The porous aluminum matrix surface is covered with irregularly shaped holes with a mean radius of about 15 microm, similar to the surface papillae of natural Lotus leaf, with villus-like nanoparticles array on pore surfaces. The superhydrophobic property is attributed to this special surface morphology and low surface energy SA. X-ray powder diffraction (XRD) pattern and Energy Dispersive X-Ray Spectroscopy (EDS) spectrum indicate that Al2O3, Al(OH)3 and AIO(OH) has been formed on the surface of aluminum substrate after the oxidation reaction. The Raman spectra indicate that C-H bond from SA and the Al-O are formed on the SAAS. The as-formed SAAS has good stability.

    An understanding of the solid-phase thermodynamics and aqueous speciation of aluminum is critical to our ability to understand and predict processes in a wide variety of geologic and industrial settings. Boehmite (AIO(OH)) is an important phase in the system Al 2 O 3 -H 2 O that has been the subject of a number of structural and thermodynamic studies since its initial synthesis [l] and discovery in nature [2]. Unfortunately, it has long been recognized that thermogravimetric analysis (TGA) of both synthetic and natural boehmite samples (that appear well crystallized by powder XRD methods) yields significant excess water - typically losing 16-16.5 wt. % on heating as compared with a nominal expected weight loss of 15.0 wt. % [3,4]. The boehmite used in our experiments was synthesized hydrothermally from acid-washed gibbsite (Al(OH) 3 ) at 200C. Powder XRD and SEM examination showed no evidence of the presence a contaminant phase. The TGA patterns do not suggest that this is due to adsorbed water, so a structural source is likely. We therefore undertook to examine this material by TEM to clarify this phenomenon

  10. Occurrence and mineralogy of ferruginous bauxites along the eastern seaboard of South Africa

    The distribution of ferruginous bauxites along the eastern seaboard of South Africa is shown in a small-scale map and their genesis is briefly described. The aluminium oxide, iron oxide, primary slightly-altered iron and titanium oxides and layer silicate minerals in some typical ferruginous bauxites from the eastern seaboard of South Africa have been investigated by X-ray powder diffraction (XRD), infra-red spectroscopy (IR), thin section, electron optical (SEM and TEM) and chemical extraction analysis. The dominant aluminium oxide mineral is well-crystalline gibbsite. Traces of boehmite were identified in some samples. The dominant iron oxide mineral is finely-divided goethite containing from 20 to 25 mole per cent AIO(OH), this being the mineral which gives the bauxites their characteristic yellowish colour. The reddish bauxites also contain finely-divided Al-substituted hematite which masks the colour of the goethite. The aluminium incorporated in the iron oxide structures is not recoverable by the normal Bayer Process

    Hintergrund: Canine Mammatumoren (CMT) sind wegen ihrer Häufigkeit und hohen Malignitätsrate eine Herausforderung für die Veterinärmedizin. Bisher ist noch keine postoperative adjuvante Therapie als wirksamer Standard etabliert und in den nächsten Jahren wohl auch nicht zu erwarten. Zusätzlich ist die Frage nach der Verträglichkeit einer adjuvanten Therapie mit Erhaltung oder Verbesserung der Lebensqualität (LQ) wichtig. Die Therapie mit Mistelextrakten (Viscum album L.; VAE) ist in der Humanonkologie nach adjuvanter Tumorbasistherapie (Chemotherapie und Bestrahlung) eine sehr häufig verwendete, zusätzliche adjuvante Behandlungsmethode. Auch bei verschiedenen Tierarten werden inzwischen Mistelpräparate in der Onkologie erfolgreich angewendet. Methoden: Überprüfung von Wirkung und Nutzen einer postoperativen, adjuvanten Misteltherapie beim CMT sowie Erfassung der LQ unter der VAE-Behandlung. Ausgewertet wurden 56 Hündinnen mit Mammaadenokarzinom, 33 ausschließlich operierte Kontrolltiere und 23 operierte Tiere, die adjuvant VAE erhielten. Ergebnisse: Die mediane Überlebenszeit (MST) aller Tiere (n = 56) betrug 32 Monate (Interquartilbereich 13-51 Monate). Im deskriptiven Vergleich der Überlebenszeiten (ST) nach Kaplan-Meier waren nach 12, 24, 36 bzw. 48 Monaten noch 24, 20, 15 bzw. 5 Hündinnen (entsprechend 72,7%, 60,6%, 45,1%, 12,4%) der Kontrollgruppe sowie 19, 14, 11 und 1 Hündin (82,6%, 60,9%, 47,8%, 4,3%) der VAE-Gruppe am Leben. Die VAE-Therapie führte zu einem geringeren Gesamtversterberisiko, das statistisch nicht signifikant war (Hazard Ratio (HR) 0,530, 95%-Konfidenzintervall (KI) 0,222-1,262; p = 0,15). Tendenziell (p = 0,07) zeigte sich eine Verringerung des tumorbedingten Sterberisikos auf 25% (HR 0,251, 95%-KI 0,056-1,122). Epikrise: Das oben beschriebene Zystadenolymphom war mit 7 cm Durchmesser ungewoehnlich gross. Durch eine Strahlentherapie mit 50 Gy kam es zu einer kompletten Tumorrueckbildung. Wie das gute klinische Ansprechen nach 30 Gy nahe legt, ist die erforderliche Dosis bei weniger ausgedehnten Zystadenolymphomen moeglicherweise niedriger.     einer mittleren Nachbeobachtungszeit von 58,2 Monaten wurden die Ueberlebensrate, die Tumorkontrolle, die Nebenwirkungen und die Kosmetik evaluiert. Lediglich ein Lokalrezidiv wurde nachgewiesen (2,3 %) bei einer statistischen 5-Jahres-Rate von 3,7 %. Keine weiteren lokoregionaeren Rezidive oder Fernmetastasen wurden beobachtet. Es verstarben 2 Patientinnen aufgrund internistischer Erkrankungen. Das krankheitsfreie-, tumorspezifische- und 5-Jahres-Gesamtueberleben war 96,3, 100 und 95,1 %. Es wurden lediglich Grad-1 Akutnebenwirkungen beobachtet: Erythem in 75 %, parenchymale Induration in 46 % und Schmerzen in 46 %, jedoch keine hoehergradigen Akuttoxizitaeten. An chronischen Nebenwirkungen Grad-1 wurden Hyperpigmentation in 12 %, Schmerzen in 2 % und Fibrosen in 44 % diagnostiziert, Letztere entwickelte sich auch hoehergradig (Grad 2 in 7 %, Grad 3 in 2 %). Asymtomatische Fettnekrosen traten in 14 % auf. Die Kosmetik wurde von 86 % der Patientinnen als ausgezeichnet/gut beurteilt, von den Aerzten in 84 %. Die lokale Kontrolle nach 5 Jahren, die Toxizitaet und die Kosmetik nach APBI mittels 3-D-konformaler Radiotherapie ist ermutigend und unsere Beobachtung entspricht den Daten anderer APBI-Veroeffentlichungen.   16. Influence of organic substrates on the kinetics of bacterial As(III) oxidation

    Soil microflora plays a major role on the behavior of metals and metalloids. Arsenic speciation, in particular, is related to the activity of bacteria able to oxidize, reduce or methylate this element, and determines mobility, bioavailability and toxicity of As. Arsenite (AsIII) is more toxic and more mobile than arsenate (AsV). Bacterial As(III)-oxidation tends to reduce the toxicity of arsenic in soils and the risk of transfer toward underlying aquifers, that would affect the quality of water resources. Previous results suggest that organic matter may affect kinetics or efficiency of bacterial As(III)-oxidation in presence of oxygen, thus in conventional physico-chemical conditions of a surface soil. Different hypothesis can be proposed to explain the influence of organic matter on As(III) oxidation. Arsenic is a potential energy source for bacteria. The presence of easily biodegradable organic matter may inhibit the As(III) oxidation process because bacteria would first metabolize these more energetic substrates. A second hypothesis would be that, in presence of organic matter, the Ars system involved in bacterial resistance to arsenic would be more active and would compete with the Aio system of arsenite oxidation, decreasing the global As(III) oxidation rate. In addition, organic matter influences the solubility of iron oxides which often act as the main pitfalls of arsenic in soils. The concentration and nature of organic matter could therefore have a significant influence on the bioavailability of arsenic and hence on its environmental impact. The influence of organic matter on biological As(III) oxidation has not yet been determined in natural soils. In this context, soil amendment with organic matter during operations of phytostabilization or, considering diffuse pollutions, through agricultural practices, may affect the mobility and bio-availability of the toxic metalloid.   17. Metagenomic analysis revealed highly diverse microbial arsenic metabolism genes in paddy soils with low-arsenic contents

    Microbe-mediated arsenic (As) metabolism plays a critical role in global As cycle, and As metabolism involves different types of genes encoding proteins facilitating its biotransformation and transportation processes. Here, we used metagenomic analysis based on high-throughput sequencing and constructed As metabolism protein databases to analyze As metabolism genes in five paddy soils with low-As contents. The results showed that highly diverse As metabolism genes were present in these paddy soils, with varied abundances and distribution for different types and subtypes of these genes. Arsenate reduction genes (ars) dominated in all soil samples, and significant correlation existed between the abundance of arr (arsenate respiration), aio (arsenite oxidation), and arsM (arsenite methylation) genes, indicating the co-existence and close-relation of different As resistance systems of microbes in wetland environments similar to these paddy soils after long-term evolution. Among all soil parameters, pH was an important factor controlling the distribution of As metabolism gene in five paddy soils (p = 0.018).     Full Text Available To attract and retain customers, an understanding of their motives and reasons for selecting particular food and grocery store is needed. This is of particular importance in the growing segment of seniors. The size of the senior's market demands a better understanding of the older consumer. The aim of our study is to identify psychographic characteristics of the elderly consumer, and to indicate the lifestyle groups and the relationship between these groups and retail store attributes. Differences in the motives for patronizing specific food stores are analyzed for lifestyle groups. We use the lifestyle as a segmentation variable in the diverse population of seniors for the reason, that the lifestyle of the elderly provides more valuable information than chronological age alone. This information can be used by retailers to improve marketing strategies in order to appeal to a target group of senior shoppers. Empirical research is based on a self-administrated questionnaire aimed on the identification of the lifestyle characteristics and retail store attributes of the consumers in 65+ age, used for the choice of food purchasing retail stores. Lifestyles characteristics were measured by the respondents activities, interests and opinions (AIO. The results of the research indicate that there are differences among the lifestyle groups with significant differences in attitudes towards quality of products or internal store environment. Our research demonstrates the value of psychographic information over age alone regarding the patronage factors in store selection.     Full Text Available O objetivo deste estudo foi de realizar avaliação da biomecânica de ciclistas, que realizaram tratamento para lombalgia, por meio do método de biofotogrametria . Foi realizado um estudo de caso-controle onde foram selecionados 9 atletas, que foram submetidos a um programa de tratamento de 10 atendimentos de aproximadamente 1 hora por meio de um protocolo de exercícios propostos pelo método McKenzie (22,56 ± 5,5anos; altura 1,76 ± 0,07 m; IMC 23,48 ± 1,76. Foram realizadas avaliações posturais e através da biofotogramtria no pré e no pós tratamento. Em relação a avaliação postural foram encontradas diferenças estatisticamente significativas, corroborando com a hipótese inicial deste estudo. Na vista anterior, em NO1, nivelamento do ombro, houve diminuição da média pré e pós tratamento, passando de 92,13° para 90,84° com p=0,038; na vista lareral em JOE, joelho, houve diminuição também de 177,97° para 171,14° com p=0,015. O ângulo RCM, relação cabeça maléolo, passou de 3,33° para 3,13° com p=0,030. Na vista posterior, o ângulo CI, coluna inferior, teve diminuição de 90,76° para 88,06° , com p=0,021; e o ângulo AIO, ângulo intraolecraniano, passou de 66,90 ° para 63,64° com p=0,028.

    Full Text Available Over the last decade, the World has shown increased concern for climate change and energy security. The emergence of these issues has pushed many nations to pursue the development of clean domestic electricity production via renewable energy (RE technologies. However, RE also comes with a higher production and investment cost, compared to most conventional fossil fuel based technologies. In order to analyse exactly how Romanian electricity consumers feel about the advantages and the disadvantages of RE, we have decided to perform a comprehensive study, which will constitute the core of a doctoral thesis regarding the Romanian energy sector and household consumers’ willingness to pay for the positive attributes of RE. The current paper represents one step toward achieving the objectives of the above mentioned research, specifically dealing with the issue of segmenting household energy consumers given the context of the Romanian energy sector. It is an argumentative literature review, which seeks to critically assess the methodology used for customer segmentation in general and for household energy users in particular. Building on the experience of previous studies, the paper aims to determine the most adequate segmentation procedure given the context and the objectives of the overall doctoral research. After assessing the advantages and disadvantages of various methodologies, a psychographic segmentation of household consumers based on general life practices is chosen, mainly because it provides more insights into consumers compared to traditional socio-demographic segmentation by focusing on lifestyles and not external characteristics, but it is also realistically implementable compared to more complex procedures such as the standard AIO.     Aim: Keeping in line with the increasing emphasis on organ preservation, we at the Tata Memorial Hospital have evaluated the role of Ir-192 interstitial implant as regards local control, functional and cosmetic outcome in early as well as locally recurrent carcinoma of the distal penis. Patients and Methods: From October 1988 to December 1996, 23 patients with histopathologically proven cancer of the penis were treated with radical radiation therapy using Ir-192 temporary interstitial implant. Our patients were in the age group of 20 to 60 years. The primary lesions were T1 and 7, T2 in 7 and recurrent in 9 patients. Only 7 patients had palpable groin nodes at presentation, all of which were pathologically negative. The median dose of implant was 50 Gy (range 40 to 60 Gy), using the LDR afterloading system and the Paris system of implant rules for dosimetry. Follow-up ranged from 4 to 117 months (median 24 months). Results: At last follow-up 18 of the 23 patients remained locally controlled with implant alone. Three patients failed only locally, 2 locoregionally and 1 only at the groin. Of the 5 patients who failed locally, 4 were successfully salvaged with partial penectomy and remained controlled when last seen. Local control with implant alone at 8 years was 70% by life table analysis. The patients had excellent functional and cosmetic outcome. We did not record any case of skin or softtissue necrosis. Only 2 patients developed meatal stenosis, both of which were treated endoscopically. Conclusion: Our results lead us to interpret that interstitial brachytherapy with Ir-192 offers excellent local control rates with preservation of organ and function. Ein Fragebogen mit 18 Fragen, basierend auf der kategorischen Einteilung des National Center for Complementary and Integrative Health, wurde entwickelt. Alle Patienten, die zwischen April und September 2015 an der Klinik fuer RadioOnkologie und Strahlentherapie des Klinikums rechts der Isar eine Strahlentherapie (RT) erhalten hatten, wurden evaluiert. Akzeptanzaenderungen der Patienten hinsichtlich CAM wurden mit einem weiteren Fragebogen nach RT bei der ersten Nachsorgeuntersuchung ermittelt (n = 31). Von 634 Patienten beantworteten 333 (52,5 %) den Fragebogen. Von allen Befragten nutzten 39,3 % CAM vor RT und 26,4 % waehrend RT. Die haeufigsten Methoden waren Vitamine/Mineralstoffe, Nahrungsergaenzungsmittel, Physiotherapie/Manuelle Medizin und Homoeopathie. Die Mehrheit (71,5 %) verneinte eine parallel laufende CAM. Grund dafuer war bei 73,5 % aller Befragten das fehlende Angebot seitens der Aerzte. Beweggruende fuer den CAM-Gebrauch waren das Immunsystem zu verbessern (43,8 %), Therapienebenwirkungen zu verringern (43,8 %) und keine Chance auszulassen (37,8 %). Eine in das Therapiekonzept integrierte Behandlung wie z. B. Akupunktur wuerden 63,7 % (212/333) der RT-Patienten in Anspruch nehmen. Im Studienvergleich zeigte sich, dass der CAM-Einsatz parallel zur RT an unserer Klinik gering ist.     This study aims to evaluate local control and intermediate-term cosmetic outcome in patients with cancer of the nose treated with intensity-modulated radiotherapy (IMRT). From June 2008 to September 2015, 36 consecutive patients presenting with nasal cavity, ala of the nose, or nasal vestibule tumors were treated at the Department of Radiation Oncology, University Hospital Zurich either postoperatively (n = 14; 3/14 with nasal ablation) or with definitive IMRT (n = 22). Of these 36 patients, 8 presented with recurrent disease after surgery only and 1/36 with N1 disease. Concurrent systemic therapy was administered in 18/36 patients (50%). Nasal follow-up (FU) imaging documentation of 13 patients with preserved organ and >6 months FU offers a pre/post IMRT FU comparison. In addition, these patients' subjective evaluation of cosmesis was assessed. Mean/median FU was 41/33 months (range 5-92 months). Salvage ablation with curative intent was undergone by 3 patients with local relapse after definitive (n = 2) and postoperative (n = 1) IMRT. The 3-year local control, ultimate local control, and overall survival rates were 90, 97, and 90 %, respectively. Subjective and objective cosmetic outcome after IMRT is very satisfying so far. IMRT for nasal tumors was found to be effective and well tolerated. Intermediate-term cosmetic results are good. Radical surgical procedures may be saved for curative salvage treatment. (orig.)     Passive water treatments based on biological attenuation can be effective for arsenic-rich acid mine drainage (AMD). However, the key factors driving the biological processes involved in this attenuation are not well-known. Here, the efficiency of arsenic (As) removal was investigated in a bench-scale continuous flow channel bioreactor treating As-rich AMD (∼30-40 mg L -1 ). In this bioreactor, As removal proceeds via the formation of biogenic precipitates consisting of iron- and arsenic-rich mineral phases encrusting a microbial biofilm. Ferrous iron (Fe(II)) oxidation and iron (Fe) and arsenic removal rates were monitored at two different water heights (4 and 25 mm) and with/without forced aeration. A maximum of 80% As removal was achieved within 500 min at the lowest water height. This operating condition promoted intense Fe(II) microbial oxidation and subsequent precipitation of As-bearing schwertmannite and amorphous ferric arsenate. Higher water height slowed down Fe(II) oxidation, Fe precipitation and As removal, in relation with limited oxygen transfer through the water column. The lower oxygen transfer at higher water height could be partly counteracted by aeration. The presence of an iridescent floating film that developed at the water surface was found to limit oxygen transfer to the water column and delayed Fe(II) oxidation, but did not affect As removal. The bacterial community structure in the biogenic precipitates in the bottom of the bioreactor differed from that of the inlet water and was influenced to some extent by water height and aeration. Although potential for microbial mediated As oxidation was revealed by the detection of aioA genes, removal of Fe and As was mainly attributable to microbial Fe oxidation activity. Increasing the proportion of dissolved As(V) in the inlet water improved As removal and favoured the formation of amorphous ferric arsenate over As-sorbed schwertmannite.     To date, no study has compared the evolution of the foramen magnum area (FMA) and the posterior cranial fossa volume (PCFV) with the degree of cranial base synchondrosis ossification. To illustrate these features in healthy children. The FMA, the PCFV, and the ossification of 12 synchondroses according to the Madeline and Elster scale were retrospectively analyzed in 235 healthy children using millimeter slices on a computed tomography scan. The mean FMA of 6.49 cm in girls was significantly inferior to the FMA of 7.67 cm in boys (P < .001). In both sexes, the growth evolved in a 2-phase process, with a phase of rapid growth from birth to 3.75 years old (yo) followed by a phase of stabilization. In girls, the first phase was shorter (ending at 2.6 yo) than in boys (ending at 4.33 yo) and proceeded at a higher rate. PCFV was smaller in girls (P < .001) and displayed a biphasic pattern in the whole population, with a phase of rapid growth from birth to 3.58 yo followed by a phase of slow growth until 16 yo. In girls, the first phase was more active and shorter (ending at 2.67 yo) than in boys (ending at 4.5 yo). The posterior interoccipital synchondroses close first, followed by the anterior interoccipital and occipitomastoidal synchondroses, the lambdoid sutures simultaneously, then the petro-occipital and spheno-occipital synchondroses simultaneously. The data provide a chronology of synchondrosis closure. We showed that FMA and PCFV are constitutionally smaller in girls at birth (P ≤ .02) and suggest that a sex-related difference in the FMA is related to earlier closure of anterior interoccipital synchondroses in girls (P = .01).     Arsenic is an element best known for its highly poisonous nature, so it is not something one would associate with being a well-spring for life. Yet discoveries made over the past two decades have delineated that not only are some microbes resistant to arsenic, but that this element's primary redox states can be exploited to conserve energy and support prokaryotic growth ('arsenotrophy') in the absence of oxygen. Hence, arsenite [As(III)] can serve as an electron donor for chemo- or photo-autotrophy while arsenate [As(V)] will serve as an electron acceptor for chemo-heterotrophs and chemo-autotrophs. The phylogenetic diversity of these microbes is broad, encompassing many individual species from diverse taxonomic groups in the Domain Bacteria, with fewer representatives in the Domain Archaea. Speculation with regard to the evolutionary origins of the key functional genes in anaerobic arsenic transformations (arrA and arxA) and aerobic oxidation (aioB) has led to a disputation as to which gene and function is the most ancient and whether arsenic metabolism extended back into the Archaean. Regardless of its origin, robust arsenic metabolism has been documented in extreme environments that are rich in their arsenic content, such as hot springs and especially hypersaline soda lakes associated with volcanic regions. Searles Lake, CA is an extreme, salt-saturated end member where vigorous arsenic metabolism occurs, but there is no detectable sulfate-reduction or methanogenesis. The latter processes are too weak bio-energetically to survive as compared with arsenotrophy, and are also highly sensitive to the abundance of borate ions present in these locales. These observations have implications with respect to the search for microbial life elsewhere in the Solar System where volcanic-like processes have been operative.   10. Surgical treatment of colorectal cancer complicated with acute intestinal obstruction

    Full Text Available Background. The main reason for urgent complications of colon cancer is an acute intestinal obstruction (AIO. This is complex pathological condition in 90 % of cases caused by colorectal cancer (CRC.Objective – to evaluate radicality of the performed operations in complicated colorectal cancer in general surgical hospitals. Dependence of the severity of intestinal obstruction by tumor localization, its morphological characteristics, determine dependence of the type of the surgical operation performed on the severity of intestinal obstruction.Materials and methods. We have studied the data on 667 patients with colorectal cancer complicated by acute intestinal obstruction. These patients were treated in the period from 2001 to 2013 in general surgical hospital in the territory of Smolensk and Smolensk region. For the processing of the obtained results we have used software Statistica 6.1. Differences were considered statistically at p ≤ 0.05.Results. All the patients were divided into 3 groups by the expression of intestinal obstruction. Group 1 (n = 279 consisted of patients with the presence of decompensated intestinal obstruction (DIO, group 2 (n = 313 consisted of patients with subcompensated intestinal obstruction (SIO, group 3 (n = 75 included patients with compensated intestinal obstruction (CIO. In case of tumor localization in right halfof the colon we most commonly observed clinical picture of acute development of decompensated intestinal obstruction (p = 0.041. Subcompensated intestinal obstruction prevailed in case of tumor localization in left half of the colon and rectal localization. In general surgical hospitals it is not always possible to speak about radicality of surgical treatment, as in a large number of cases (62.5 % the number of examined lymph nodes was less than 4.     Superconducting YBa 2 Cu 3 O x thin films were produced by in situ off-axis RF magnetron sputter deposition using a stoichiometric single target (T1). The effects of individual deposition parameters on the film properties are discussed. The transition temperatures for films deposited on MgO (100) are slightly lower than that for the SrTiO 3 and LaAlO 3 (100)-oriented substrates. The lower transition temperatures in the MgO substrates are most likely the result of the larger lattice mismatch with YBa 2 Cu 3 O x . All films grown on (100) substrates are c-axis oriented. The orientation of the sputtered YBCO films appears to depend only on the substrate orientation and not on the substrate material choice. Typical results for the c-axis lattice parameter c for various YBCO thin films sputtered onto SrTiO 3 , LaAIO 3 and MgO all (100)-oriented substrates with a range of T c values are presented. Three different targets were used in the course of this work: the first one (T1) was a 60 mm diameter, 3 mm thickness high density commercial YBCO disc; the second one (T2) was a 35 mm diameter, 4 mm thickness high density YBCO disc; and the third one (T3) was a 60 mm diameter, 3 mm thickness YBCO powder pressed then sintered on a copper plate substrate. The latter two were manufactured in house and are reported here for the first time. The best results were obtained using T1. The best films had T c (onset)= 89-93 K close to that of bulk YBCO and a transition width ΔT c =2-5 K. But some other films exhibited lower critical temperatures T c (onset)= 62-70 K and broader transitions ΔT c = 10.4-13.6 K. These reductions in T c and the broadening of the transition are due to oxygen deficiency. This is shown by the c-axis lattice parameter calculations. All films grown using T2 and T3 exhibited lower critical temperatures and broader transitions for the same reason.
