    Admission of an infant to the neonatal intensive care unit (NICU) is a stressful experience for parents. Parents' education improves knowledge and satisfaction. The purpose of this study was to evaluate the influence of using a family information guide about the NICU as a communication intervention between the healthcare providers and parents having their infants in the unit. An interventional study was conducted among 100 fathers with their neonates inside the NICU-Kasr Al Ainy Teaching Hospital, Cairo, Egypt. The study passed through three stages: pre-intervention, intervention using a family information guide, and post-intervention. After using the guide, fathers showed significant improvement in their knowledge, with a change in their feelings towards the admission of their neonates to the NICU. Providing sufficient information and increasing awareness of parents about the NICU in the form of a written guide is an effective way of improving communication between healthcare providers and parents.     Full Text Available ABSTRACT The increasing volume of garbage of Balinese Hindu’s offering is still a problem. The garbage, when it is decomposed however could be useful for fertilized. This organic litter can be decomposed in biopore. This research ainied to investigated the decomposition rate of C. nucifera and O. tigillarium litter in biopores. This research was conducted between February and July 2015. Garbage were collected from temples in Jembrana city and treated at Dangin Tukadaya village, Jembrana Distric. Sampels were arranged in a Factorial Randomized Blocked Sampling Design 2 factors i.e: treatments and times (1, 2, 3, 4 and 23 weeks. The result showed that decomposition rate 98,63% of cocos litter and 99,23% of oncosperma litter were decomposed in biopore on week 23 compared to 89,06% of cocos and 75,76 % of oncosperma litter in control. Decomposition mean rate is 9,33 %/week of cocos litter and 9,64 %/week of oncosperma litter were decomposed in biopore but in control is 7,85 %/week of cocos and 8,07 %/week of oncosperma litter. Total cocos and oncosperma litter decomposition took about 23,3 and 23,2 weeks in biopore but for control 25,8 and 30,4 weeks The number of millipedes (Harpaphe haydeniana as decomposer was found higher compared to other decomposers such as earth worms and molusca.     To evaluate a new technique combining intrastromal astigmatic keratotomy (AK) with a laser in situ keratomileusis (LASIK) flap followed by excimer laser photoablation to correct post-penetrating keratoplasty (PKP) high astigmatism and ametropia. Kasr El Aini Hospital, Cairo University, Cairo, Egypt. Prospective interventional uncontrolled case series. Patients with post-PKP high astigmatism and ametropia had paired intrastromal AK with LASIK flap using the M2 microkeratome followed 2 to 3 months later by excimer laser photoablation. The main outcome measures were uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), mean refractive spherical equivalent (SE), and mean cylinder after each step and at the 1-year follow-up. The study comprised 20 eyes (20 patients). All parameters were significantly improved in all patients by the last follow-up visit. The mean UDVA improved from 1.07 logMAR ± 0.2 (SD) preoperatively to 0.23 ± 0.18 logMAR (P astigmatism and ametropia following PKP; however, epithelial ingrowth requiring intervention is a complication to be considered.     This study aimed at evaluating possible associations of the single nucleotide polymorphism (SNP) in luteinizing hormone/choriogonadotropin receptor (LHCGR) gene G935A and polycystic ovary syndrome (PCOS) phenotype. The study included 100 PCOS female patients and 60 healthy female control subjects. The patients were recruited from the Gynecology out-patient clinic, Kasr Al-Aini Hospital, Cairo University. All candidates underwent full history taking and clinical examination with calculation of body mass index. Serum and EDTA samples were collected from each patient after a written consent. A hormonal profile was done for each patient as well as DNA analysis of the G935A polymorphism of LHCGR gene. In PCOS group, 26% were homozygous (AA), 27% were heterozygous (GA) and 47% were wild genotype (GG), while in controls 30% were heterozygous and 70% were wild genotype (OR: 2.25; CI: 1.16-4.386; p value: 0.012). The homozygous 935A individuals were at higher risk to develop PCOS than controls (OR: 1.80; CI: 1.54-2.09; p value genetic variant, which is associated with PCOS in a sample of the Egyptian population. These results may provide an opportunity to test this SNP at the LHCGR gene in fertile or infertile women with family history to assess their risk of PCOS.

    To examine the histological structure and vascularity of uterine septa in patients with unexplained primary infertility and patients with recurrent pregnancy loss (RPL). Case-control study (Canadian Task Force classification II-2). Kasr Al Aini Hospital, Cairo University, Egypt. A total of 41 patients with uterine septa presenting with either unexplained primary infertility or RPL underwent 3-dimensional (3D) and power Doppler (PD) ultrasound to calculate the septum volume, septal vascularization index (VI), and myometrial VI. Hysteroscopically removed septa were examined histologically for myometrial and fibrous tissue content. Septal characteristics differed between the primary infertility group (24 patients) and the RPL group (17 patients). Mean septal VI was significantly higher in the RPL group compared with the infertility group (18.39% ± 7.57% vs 11.67% ± 4.56%; p = .003), as was septal myometrial area (20.74% ± 5.97% vs 13.57% ± 5.55%; p infertility.   18. Study of the newborn feeding behaviors and fentanyl concentration in colostrum after an analgesic dose of epidural and intravenous fentanyl in cesarean section.

    To compare the effects of epidural and intravenous fentanyl on breast feeding behaviors and fentanyl concentration in the colostrum after an analgesic dose. This study was conducted at the Obstetrics Department of Kasr El-Aini Hospital-Cairo University, Cairo, Egypt. The studied mothers were 100 multipara, who have been subjected to cesarean section, and have a previous history of successful breast feeding. The study was conducted from May 2005 to May 2007. They were divided into 2 groups: group I included 50 patients who received epidural anesthesia with fentanyl, and group II included 50 patients who received spinal anesthesia with intravenous fentanyl, and both groups were observed for initial breast feeding behaviors of newborns, and fentanyl concentration in the colostrum at 45 minutes, and 24 hours after birth. The study included 100 multipara, 2 samples of colostrum were taken from each patients at 45 minutes, and at 24 hours. The levels of fentanyl concentration were greatest at 45 minutes of the initial sampling time, reaching 0.40+/-0.059 ng/ml in the epidural group, and 0.19+/-0.019 ng/ml in intravenous fentanyl group. There was no statistical difference in breast feeding behaviors at birth, or at 24 hours of age in both groups. Although the levels of fentanyl concentration were greatest at 45 minutes of the initial sampling time, it can be used safely as intravenous or epidural without affecting the initial breast feeding behaviors of the newborn.

    A taxonomic revision of the subgenus Chthonius (Ephippiochthonius) in the Iberian Peninsula, Balearic Islands and    Macaronesia is presented. New and previously insufficiently studied characteristics are shown to be taxonomically useful, such as the chelal lyrifissures patterns, chaetotaxy and condylar complex. Three taxa previously treated as subgenera of Chthonius are here raised to generic rank: Ephippiochthonius n. stat., Globochthonius n. stat., and Hesperochthonius n. stat. Two new genera are described: Cantabrochthonius n. gen. and Occidenchthonius n. gen. Thirty-five new species are described: Ephippiochthonius aini n. sp., E. andalucia n. sp., E. aurouxi n. sp., E. benimaquia n. sp., E. caceresi n. sp., E. castellonensis n. sp., E. comasi n. sp., E. fadriquei n. sp., E. galcerani n. sp., E. gonzalezi n. sp., E. henderickxi n. sp., E. ibiza n. sp., E. masoae n. sp., E. portugalensis n. sp., E. riberai n. sp., E. serengei n. sp., E. sevai n. sp., E. tarraconensis n. sp., E. vicenae n. sp., E. zaballosi n. sp., Occidenchthonius anae n. sp., O. beieri n. sp., O. ebo n. sp., O. felgueraorum n. sp., O. gardinii n. sp., O. hoerwegi n. sp., O. lencinai n. sp., O. mahnerti n. sp., O. mateui n. sp., O. montagudi n. sp., O. murcia n. sp., O. oromii n. sp., O. ortunoi n. sp., O. riopar n. sp. and O. serranoi n. sp.   20. New attempt using ablative curettage technique for managing benign premenopausal uterine bleeding.

    To assess the efficacy of a newly suggested ablative curettage technique in managing premenopausal dysfunctional uterine bleeding. A prospective controlled operative study and clinical trial was carried out at the Department of Obstetrics and Gynecology, Kasr El-Aini Hospital, Cairo University, Egypt. One hundred cases of premenopausal bleeding were divided into 50 controls and 50 studied cases. Routine diagnostic and follow-up techniques for managing premenopausal bleeding were carried out. Two operative techniques were compared: overcurettage, using Sims sharp curette, and the newly suggested dilatation and ablative curettage, using a Thabet double-bladed sharp curette. Histopathology of curetted material and the endomyometrium after ablation in the failed cases that needed hysterectomy was also included. Successful endometrial ablation and control of the premenopausal bleeding were recorded in 66% of overcurettage cases and in 90% of cases treated with ablative curettage. The latter had significantly fewer operative and postoperative complications. These advantages were also significantly reduced in both techniques compared with other ablative techniques and hysterectomy. Complete ablation could be confirmed histopathologically in the ablated endometrium and in the endomyometrium of the hysterectomy specimens. Ablative curettage is an efficient technique in managing premenopausal dysfunctional uterine bleeding, especially in developing countries where there are limited financial resources.

Because the ethnic groups in Yunnan province used primitive and tough tea leaves, which were eaten instead of being infused in water, the leaves were first fermented by being buried in the ground. Even today, people of these ethnic groups prefer fungus-fermented black tea with a particular flavor. In contrast, the ethnic Hans used and still use improved and softer young shoots of tea leaves to prepare mainly green tea. It has recently been discovered that Acapsia, as well as Gambir, has anti-oxidant properties, and that consumption over time is effective against many lifestyle-related adult diseases. It may be well worthwhile to cast fresh light upon ancient tea drinking customs.

  3. Implementation, barriers and challenges of smoke-free policies in hospitals in Egypt

    Full Text Available Abstract Background Tobacco use is a serious public health challenge in North Africa, and health professionals play a vital role in tobacco control. In Egypt, limited data are available on the knowledge and attitudes of health care providers regarding tobacco control policies. Such data are especially relevant due to Egypt’s tobacco control laws, adopted in 2007, prohibiting smoking in hospitals and other public places. This study surveyed 49 senior administrative staff, 267 physicians, 254 nurses, and 109 administrative employees working in El-Kasr El-Aini Hospital in Cairo, assessing their knowledge and attitudes regarding Egypt’s tobacco control laws and barriers to their effective implementation in health care facilities. We also investigated the hospital’s compliance with smoke-free policies. Results The majority (>90% of the hospital workers knew that exposure to second-hand smoke is harmful to health. Physicians and nurses had a more favorable attitude towards the smoking ban when compared to administrative employees. Hospital staff identified the following barriers to successfully implementing the smoking ban: lax enforcement of tobacco control laws, the lack of penalties for violators, the lack of cessation programs, and the prevalence of smoking among physicians. Conclusions Overall, smoke-free policies were poorly enforced in this large teaching hospital in Cairo, Egypt. Interventions to address the identified barriers to their implementation could include the provision of cessation training and services as well as effective communication programs to educate health care workers at all levels regarding the dangers of second-hand smoke exposure and effective measures for protection.

    Thermal ablative therapies continue to be favored as a safe and effective treatment for patients with non-resectable hepatocellular carcinoma (HCC). Percutaneous microwave ablative therapy, which is a relatively new technique, has the advantage in providing faster ablation of large tumors. This study aimed to evaluate microwave ablation in the treatment of large HCC (5-7 cm) and to assess its effect on local tumor progression, prognostic outcome and patients' survival. In all, 26 patients with large HCC lesions (5-7 cm) were managed in the multidisciplinary clinic of Kasr Al Ainy University hospital using microwave ablation. The treatment was performed with the patient under conscious sedation and analgesia and ultrasonography-guided using an HS AMICA microwave machine operating at frequency of 2450 MHz and a power up to 100 W. Multiple needle insertions were made in one or two sessions according to the size of the lesion. The complete ablation rate, local tumor progression and patients' overall survival were analyzed, and the efficacy and safety of MWA was evaluated. Complete ablation was achieved in 19/26 (73.1%). Local tumor progression was recorded in five treated lesions (19.2%). Distant tumor progression within the liver was recorded in six patients (23.1%), with a mean survival of 21.5 months. No procedure-related major complications or deaths were observed. Percutaneous microwave ablation is safe and effective in the treatment of large HCC tumors.     Full Text Available ABSTRAK Tujuan penelitian ini adalah untuk mengetahui cemaran baku mutu air limbah Rumah Potong Hewan (RPH Sapi dan Ayam di Kota Mataram. Semua hasil melebihi baku mutu kecuali nilai pH. Kandungan ALT rerata air limbah RPH sapi 284004 x 104 koloni/ml dan mengandung mikroba jenis E.coli dan Salmonella. Sedangkan air limbah RPH ayam ALT rerata 101596 x 105koloni/ml dan mengandung kuman jenis E. coli, Pseudomonas aeruginosa dan Klebsiella pneumoniae. Tingginya kandungan BOD, COD, TSS, ammonia, pH, minyak dan lemak maupun cemaran mikroba diduga disebabkan tidak difungsikannya Instalasi Pengolahan Limbah (IPAL pada RPH sapi dan tidak optimalnya fungsi IPAL di RPH ayam.hubungan antara penerapan Adiwiyata dengan pembentukan pengetahuan, sikap dan tindakan di kalangan siswa.     Specific language impairment is a relatively common developmental condition in which a child fails to develop language at the typical rate despite normal general intellectual abilities, adequate exposure to language, and in the absence of hearing impairments, or neurological or psychiatric disorders. There is much controversy about the extent to which the auditory processing deficits are important in the genesis specific language impairment. The objective of this paper is to assess the higher cortical functions in children with specific language impairment, through assessing neurophysiological changes in order to correlate the results with the clinical picture of the patients to choose the proper rehabilitation training program. This study was carried out on 40 children diagnosed to have specific language impairment and 20 normal children as a control group. All children were subjected to the assessment protocol applied in Kasr El-Aini hospital. They were also subjected to a language test (receptive, expressive and total language items), the audio-vocal items of Illinois test of psycholinguistic (auditory reception, auditory association, verbal expression, grammatical closure, auditory sequential memory and sound blending) as well as audiological assessment that included peripheral audiological and P300 amplitude and latency assessment. The results revealed a highly significant difference in P300 amplitude and latency between specific language impairment group and control group. There is also strong correlations between P300 latency and the grammatical closure, auditory sequential memory and sound blending, while significant correlation between the P300 amplitude and auditory association and verbal expression. Children with specific language impairment, in spite of the normal peripheral hearing, have evidence of cognitive and central auditory processing defects as evidenced by P300 auditory event related potential in the form of prolonged latency which indicate a

Hormone replacement therapy has been used to improve quality of life and prevent complications, but side effects limit its use. In this study, we evaluated the use of edible bird’s nest (EBN for prevention of cardiometabolic problems in rats with ovariectomy-induced menopause. Ovariectomized female rats were fed for 12 weeks with normal rat chow, EBN, or estrogen and compared with normal non-ovariectomized rats. Metabolic indices (insulin, estrogen, superoxide dismutase, malondialdehyde, oral glucose tolerance test, and lipid profile were measured at the end of the experiment from serum and liver tissue homogenate, and transcriptional levels of hepatic insulin signaling genes were measured. The results showed that ovariectomy worsened metabolic indices and disrupted the normal transcriptional pattern of hepatic insulin signaling genes. EBN improved the metabolic indices and also produced transcriptional changes in hepatic insulin signaling genes that tended toward enhanced insulin sensitivity, and glucose and lipid homeostasis, even better than estrogen. The data suggest that EBN could meliorate estrogen deficiency-associated increase in risk of cardiometabolic disease in rats, and may in fact be useful as a functional food for the prevention of such a problem in

To date, the information regarding the association between nutritional status and physical performance does not provide a complete picture. Most studies used limited or self-reported measures to evaluate physical performance. The objective of this study was to examine the correlation between nutritional status and comprehensive physical performance measures among undernourished older adults who reside in residential institutions.Methods: Forty-seven older adults (26 males, 21 females aged ≥60 (69.23±8.63 years who were identified as undernourished from two residential institutions participated in this study. A battery of physical performance tests (10 m gait speed test, dominant hand grip strength test, timed five-repetition sit-to-stand test, ten step test, arm curl test, scratch test, and respiratory muscle strength test, biochemical profiles (serum albumin, hemoglobin, serum ferritin, and prealbumin levels, and falls risk using the short-form Physiological Profile Approach were performed. The Functional Ability Questionnaire and Geriatric Depression Scale were also administered.Results: The results demonstrated that generally older adults with undernourishment scored poorly on the physical performance tests, had depression, and a high risk of falls.   9. New findings and concepts about the G-spot in normal and absent vagina: precautions possibly needed for preservation of the G-spot and sexuality during surgery.

    The aim of this study was to extend the clinicohistological study to involve the whole normal and absent vagina for confirming the presence of the G-spot and its relation to the surrounding organs and sexuality and to identify certain precautions for its preservation during surgery. This study was a descriptive randomized prospective study conducted at Kasr El Aini School of Medicine, Cairo University, Egypt. The G-spot was examined in 1500 women, 500 of them having vaginal and vulval surgery done for gynecological reasons. The G-spot was examined for its clinical and histological features and for determining the effect of surgery on its state and function. The G-spot was found to be present in all women. It was a localized spot in 58% and diffuse in 42% of cases. Associated ejaculation was reported in all cases of the localized type and in 24.5% of the diffuse types. Clinical examination was found to be associated with certain local response in 52.7% of the local types. The G-spot was also found to be connected to the hymen in 100%, the urethra in 52.7%, the vulva in 82.2% and the cervix in 10.8% of cases. The mean of the sex scores and sexuality were significantly decreased in surgery involving the G-spot area. Recorded figures were 93.6 ± 3.4 and 88.2 ± 3.3 before and after surgery, respectively. The corresponding figures in the cases having a general spot were 86.4 ± 4.4 and 84.5 ± 2.4, respectively. The G-spot was found in cases of absent vagina to be localized in 59%, generalized in 28.2% and absent in 12.8% of cases. The G-spot is actually present in all women. It is originally related to the lower urinary tract and it is connected to different parts of the genital tract. It may be localized or generalized. Its integrity is essential for obtaining normal physiological sexuality. Surgery may affect the integrity of the G-spot, so surgical precautions must be carried out to maintain the integrity of this spot and the patient's sexuality.

  11. A Novel Endometriosis Inducing Factor In Women with Endometriosis

    Methods: The study included thirty infertile women undergoing diagnostic laparoscopy for infertility work out. Fifteen cases served as a control group where the diagnostic laparoscopy showed no endometriotic implants. The other fifteen cases had endometriosis of variable degrees based on the modified American Fertility Society (mAFS classification25. The study group was subdivided into 3 sub-groups of 5 women each, with mild, moderate and severe endometriosis. No women had history of immunological diseases or received any type of hormonal therapy in last six months before the diagnostic laparoscopy procedure. Venous blood samples were obtained and left to clot at 37°C, centrifuged and serum was separated and kept in sterile tubes at -20°C till later use.Stem cell preparation: Human umbilical cord blood (UCB was collected from mothers delivering at full term. Informed consent was obtained from all mothers in accordance with the Ethical Committee of National Research Centre guidelines, Research Ethical Committee of Kasr El-Aini teaching hospital and Islamic conference guidelines on stem cell research. In each sample, UCB was harvested in sterile tubes containing 100 mM EDTA as anticoagulant at 22°C and the low-density mononuclear cells were isolated using Ficoll-Plaque Plus (Amersham Biosciences, Sweden. Then, the cells were co-cultured in growth medium (Dulbecco’s Modified Eagle medium-low glucose with the addition of 10% fetal bovine serum with 2 mmol/l L-glutamine and 0.3% penicillin-streptomycin (Gibco- BRL, USA. Co-cultures were incubated at 37°C and 5% CO2 concentration. The mesenchymal stem cells (MSCs derived from UCB were propagated and characterized. Follow-up: Ten µL serum samples from the different subgroups were added to the stem cell culture medium at 37°C26 and followed up weekly for 4 weeks for possible differentiation of stem cells into endometrial like cells.
