    Full Text Available Objective: To compare audiometric results between the standard total ossicular replacement prosthesis (TORP-S and a new fat interposition total ossicular replacement prosthesis (TORP-F in pediatric and adult patients and to assess the complication and the undesirable outcome. Study design: This is a retrospective study. Methods: This study included 104 patients who had undergone titanium implants with TORP-F and 54 patients who had undergone the procedure with TORP-S between 2008 and 2013 in our tertiary care centers. The new technique consists of interposing a fat graft between the 4 legs of the universal titanium prosthesis (Medtronic Xomed Inc, Jacksonville, FL, USA to provide a more stable TORP in the ovale window niche. Normally, this prosthesis is designed to fit on the stapes’ head as a partial ossicular replacement prosthesis. Results: The postoperative air-bone gap less than 25 dB for the combined cohort was 69.2% and 41.7% for the TORP-F and the TORP-S groups, respectively. The mean follow-up was 17 months postoperatively. By stratifying data, the pediatric cohort shows 56.5% in the TORP-F group (n = 52 compared with 40% in the TORP-S group (n = 29. However, the adult cohort shows 79.3% in the TORP-F group (n = 52 compared with 43.75% in the TORP-S group (n = 25. These improvements in hearing were statistically significant. There were no statistically significant differences in the speech discrimination scores. The only undesirable outcome that was statistically different between the 2 groups was the prosthesis displacement: 7% in the TORP-F group compared with 19% in the TORP-S group ( P  = .03. Conclusions: The interposition of a fat graft between the legs of the titanium implants (TORP-F provides superior hearing results compared with a standard procedure (TORP-S in pediatric and adult populations because of its better stability in the oval window niche.

    The efficiency of titanium middle ear prosthesis for ossicular reconstruction in chronic ear disease is investigated in a Scandinavian two-center retrospective study from a Norwegian tertiary otology referral center and a Finnish otology referral center. Retrospective chart reviews were performed for procedures involving 73 titanium prostheses between 1999 and 2004. All patients that underwent surgery including the Kurz Vario titanium prosthesis were included in the study, 38 procedures including the partial ossicular replacement prosthesis (PORP) and 35 procedures including the total ossicular replacement prosthesis (TORP). Mean follow-up was 14 months. The ossiculoplasty was performed alone (29 patients) or in combination with other chronic ear surgery procedures (34 patients). Comparisons of preoperative and postoperative pure tone averages (0.5, 1, 2, and 3 kHz) according to AAO-HNS guidelines are presented, as well as data for different PTA definitions. Otosurgery procedures, complications, revisions, and extrusion rates are reported. A postoperative air-bone gap (ABG) of Torp) prosthesis. Overall mean pure tone averages improved 20.6 dB with ABG improvement of 19.3 dB. The overall extrusion rate was 5% (4 patients). Titanium prostheses have been easy and fast to handle and effective implants for reconstruction of the ossicular chain. We found no difference between reconstruction with or without cholesteatoma surgery during the same procedure. The combination of CWD and Torp gave significant inferior hearing thresholds as compared to Torp/CWU and Porp/CWD combinations.

    The primary objective was to report on experiences regarding the safety and efficacy of the Vibrant Soundbridge (VSB) using a floating mass transducer (FMT)-partial/total ossicular replacement prosthesis (PORP/TORP) assembly as a treatment for conductive and mixed hearing losses of mild-to-moderate/severe degree. The secondary objective was to gather information regarding device fitting, as well as to refine surgical procedures. Five German-speaking adults from 2 European study sites were implanted with a VSB using an FMT-PORP/TORP assembly and evaluated before and after surgery for air-and bone-conduction thresholds and speech recognition performance. Evaluating the safety and efficacy of the VSB in combination with a PORP or TORP to treat conductive and mixed hearing loss. Residual cochlear hearing was unaffected by implantation with the device. Functional gain (measured as the difference between preoperative unaided and postoperative VSB-aided thresholds) could be calculated in 2 of 5 subjects, demonstrating that the VSB is effective in treating bone-conduction hearing losses of moderate/severe degree. Word recognition tests in quiet and noise showed good improvement in 3 of the cases. One patient experienced several other medical problems, making her audiological outcomes limited. One patient was excluded from the study owing to insufficient benefit and subsequently underwent revision surgery with FMT placement at the round window. The use of the VSB, implanted using the FMT-PORP/TORP assembly, was safe in all and efficacious in 3 of the 5 cases in this study. These are patients who may have few, if any, other options to manage their hearing loss.

Plastipore prostheses have been found to be useful, and their continued use is advocated.     After middle ear reconstruction using partial or total ossicular replacement prostheses (PORP/TORP), an air-bone gap (ABG) may persist because of prosthesis displacement or malposition. So far, CT of the temporal bone has played the main role in the diagnosis of reasons for postoperative insufficient ABG improvement. Recent experimental and clinical studies have evaluated flat panel CT (fpCT) as an alternative imaging technique that provides images with high isovolumetric resolution, fewer metal-induced artefacts and lower irradiation doses. One hundred and seven consecutive patients with chronic otitis media with or without cholesteatoma underwent reconstruction by PORP (n = 52) or TORP (n = 55). All subjects underwent preoperative and postoperative audiometric testing and postoperative fpCT. Statistical evaluation of all 107 patients as well as the sole sub-assembly groups (PORP or TORP) showed a highly significant correlation between hearing improvement and fpCT-determined prosthesis position. FpCT enables detailed postoperative information on patients with middle ear reconstruction. FpCT is a new imaging technique that provides immediate feedback on surgical results after reconstructive middle ear surgery. Specific parameters evaluated by fpCT may serve as a predictive tool for estimated postoperative hearing improvement. Therefore this imaging technique is suitable for postoperative quality control in reconstructive middle ear surgery. (orig.)

    Full Text Available Background: Bone conduction (BC threshold depression is not always by means of sensory neural hearing loss and sometimes it is an artifact caused by middle ear pathologies and ossicular chain problems. In this research, the influences of ear surgeries on bone conduction were evaluated. Materials and Methods: This study was conducted as a clinical trial study. The ear surgery performed on 83 patients classified in four categories: Stapedectomy, tympanomastoid surgery and ossicular reconstruction partially or totally; Partial Ossicular Replacement Prosthesis (PORP and Total Ossicular Replacement Prosthesis (TORP. Bone conduction thresholds assessed in frequencies of 250, 500, 1000, 2000 and 4000 Hz pre and post the surgery. Results: In stapedectomy group, the average of BC threshold in all frequencies improved approximately 6 dB in frequency of 2000 Hz. In tympanomastoid group, BC threshold in the frequency of 500, 1000 and 2000 Hz changed 4 dB (P-value < 0.05. Moreover, In the PORP group, 5 dB enhancement was seen in 1000 and 2000 Hz. In TORP group, the results confirmed that BC threshold improved in all frequencies especially at 4000 Hz about 6.5 dB. Conclusion: In according to results of this study, BC threshold shift was seen after several ear surgeries such as stapedectomy, tympanoplasty, PORP and TORP. The average of BC improvement was approximately 5 dB. It must be considered that BC depression might happen because of ossicular chain problems. Therefore; by resolving middle ear pathologies, the better BC threshold was obtained, the less hearing problems would be faced.

    Full Text Available Background/Aim. This paper presents our operative method for hearing recovery after the previous radical tympanomastoidectomy, radical trepanation of the temporal bone (trepanatio radicalis ossis temporalis - TROT in eight patients submitted to operations for giant cholesteatotoma. Methods. All the patients were admitted to our clinic after TROT. There were no signs of cholesteatoma or infection. The patients refused any stent implantations or any hearing aids due to possible aesthetic problems. The described procedure developed in two steps. The first one was to restore the destroyed cavum tympany and to covert with chondroperichondral new membrane with a pin-like “guide” as collumela. The second step was to insert a TORP (total ossicular replacement prosthesis after guide excision. Results. After the first operation (stage one there were no infections in the operated area nor chondroperichondral graft rejection. Postoperative audiometry (6 to 8 weeks was done to demonstrate the improvement of air conduction. Three months following the first, the second (stage two operation was performed and 2.5 to 3 months after this operation even greater audiometry revealed hearing improvement in air- and bone-conduction. The patients were dismissed from the hospital 2 days after each procedure without any complications. They did not experience any dizziness, vomiting nor a severe pain. Three months after the second operative stage, otoscopic findings were very good. The audiometry findings after a 3-months period (after stage one and 3 months after final TORP insertion was done for each of the patients. After one year, the audiometric curve was the same. Clinical and audiometry follow up demonstrated a hearing recovery and closure of air bone gap (ABG to values of 5 to 15 dB. Conclusion. The use of TORP after radical tympanomastoidectomy is feasible.     Titanium implants in middle ear surgery were introduced in the late 90s and are now frequently used in middle ear surgery. However, long-term studies of patient outcome are few and have only been published in subgroups of patients. We report the long-term effect of titanium middle ear implants for ossicular reconstruction in chronic ear disease investigated in a Norwegian tertiary otological referral centre. Retrospective chart reviews were performed for procedures involving 76 titanium implants between 2000 and 2007. All patients who underwent surgery using the Kurz Vario titanium implant were included in the study. Audiological parameters using four frequencies, 0.5, 1, 2, and 3 kHz, according to AAO-HNS guidelines, was assessed pre and postoperatively. Otosurgical procedures, complications, revisions, and extrusion rates were analyzed. The study had no dropouts. The partial ossicular replacement prosthesis (PORP) was used in 44 procedures and the total ossicular replacement prosthesis (TORP) in 32 procedures, respectively. Mean follow-up was 5.2 years (62 months). The ossiculoplasties were performed as staging procedures or in combination with other chronic ear surgery. The same surgeon performed all the procedures. A postoperative air-bone gap of ≤ 20 dB was obtained in 74 % of the patients, 82 % for the Bell (PORP) prosthesis, and 63 % for the Arial (TORP) prosthesis. The extrusion rate was 5 %. We conclude that titanium ossicular implants give stable and excellent long-term hearing results.

    To explore the technique of hearing reconstruction in the cases of conductive hearing loss with malformed tympanic segment of facial nerve. Data of 10 cases from July 2010 to March 2015 were collected.The status of tympanic segment of facial nerve, malformed ossicles and the reconstructed methods of ossicular chain were analyzed and discussed based on the embryo anatomy and surgical technique. All facial nerves in 10 cases were exposed and drooping to stapes or cover the oval window.Three patients who had normal stapes, pushed by the exposed facial nerve, were reconstructed with partial ossicular replacement prostheses (PORP). Two patients who had footplate, with partial fixation, were reconstructed with total ossicular replacement prostheses (TORP). Three patients who had atresia of the oval window were implanted with Piston after being made hole in the atresia plate.Another two cases who had atresia of the oval window were implanted with TORP after promontory being drilled out.All cases had no injury of facial nerve and nervous hearing, and no tinnitus.Nine cases had conductive hearing improvement, except one with promontory drilled out. Patients who had conductive hearing loss with malformed tympanic segment of facial nerve can be treated by the technique of hearing reconstruction.The fenestration technique in the bottom of the scala tympani of the basal turn provides us a new method for treating patients whose oval window was fully covered by malformed facial nerve.

    Full Text Available Introduction: The use of ossicular graft material in ossicular chain reconstruction has significantly improved hearing results hearing after tympanoplasty and tympanomastoid surgery for chronic otitis media. Today, otologists have a wide array of tools from which to choose, but may find it difficult to know which middle ear implant works best.   Materials and Methods: A prospective study of 80 patients who underwent ossiculoplasty was performed in the ear, nose, and throat (ENT department at a tertiary health care facility from 2011 to 2013. Patients with chronic suppurative otitis media with an air-bone gap (ABG of >25 dB with ossicular involvement were included in the study. Total ossicular replacement prosthesis (TORP, partial ossicular replacement prosthesis (PORP, and refashioned incus were used. Success was defined as ABG   Results: The majority patients were of middle age with moderate conductive hearing loss. Incus was the most susceptible ossicle. Overall success rate in this study was 80.0% with an average change of 15.76 dB in ABG.   Conclusion:  With continuing advances in our understanding of middle ear mechanics, the results of ossiculoplasty are improving and results can be very rewarding in experienced hands. Severity of preoperative ear discharge, preoperative mastoid cellularity, presence of disease, and surgical procedure proved to be significant prognostic factors. Autograft incus and PORP fared better when the malleus handle was present while TORP gave better results when the malleus handle was eroded.

    This report describes the development and testing of the Kansas Technology Transfer Model (KTTM) which is to be utilized as a regional model for the development of other technology transfer programs for independent operators throughout oil-producing regions in the US. It describes the linkage of the regional model with a proposed national technology transfer plan, an evaluation technique for improving and assessing the model, and the methodology which makes it adaptable on a regional basis. The report also describes management concepts helpful in managing a technology transfer program. The original Tertiary Oil Recovery Project (TORP) activities, upon which the KTTM is based, were developed and tested for Kansas and have proved to be effective in assisting independent operators in utilizing technology. Through joint activities of TORP and the Kansas Geological Survey (KGS), the KTTM was developed and documented for application in other oil-producing regions. During the course of developing this model, twelve documents describing the implementation of the KTTM were developed as deliverables to DOE. These include: (1) a problem identification (PI) manual describing the format and results of six PI workshops conducted in different areas of Kansas, (2) three technology workshop participant manuals on advanced waterflooding, reservoir description, and personal computer applications, (3) three technology workshop instructor manuals which provides instructor material for all three workshops, (4) three technologies were documented as demonstration projects which included reservoir management, permeability modification, and utilization of a liquid-level acoustic measuring device, (5) a bibliography of all literature utilized in the documents, and (6) a document which describes the KTTM.

    Numerous synthetic ossicular replacements are currently in use. The TORP conducts sound from the newly formed tympanic membrane to the oval window; the PORP is used when the stapes superstructure is maintained, being interposed between the tympanic membrane and the stapes capitulum. In 12 patients the surgical results of ossicular replacement procedures were good, which gave the author the opportunity to study the normal CT appearance. In an additional 10 patients CT was performed before surgical revision. Using CT, they have been able to diagnose subluxation and fibrous tissue fixation. In two patients the CT appearance was unremarkable, but at surgery lateralization of the graft was found, with a nonfunctioning interface

  20. Estimation of land-atmosphere energy transfer over the Tibetan Plateau by a combination use of geostationary and polar-orbiting satellite data

    Land-atmosphere energy transfer is of great importance in land-atmosphere interactions and atmospheric boundary layer processes over the Tibetan Plateau (TP). The energy fluxes have high temporal variability, especially in their diurnal cycle, which cannot be acquired by polar-orbiting satellites alone because of their low temporal resolution. Therefore, it's of great practical significance to retrieve land surface heat fluxes by a combination use of geostationary and polar orbiting satellites. In this study, a time series of the hourly LST was estimated from thermal infrared data acquired by the Chinese geostationary satellite FengYun 2C (FY-2C) over the TP. The split window algorithm (SWA) was optimized using a regression method based on the observations from the Enhanced Observing Period (CEOP) of the Asia-Australia Monsoon Project (CAMP) on the Tibetan Plateau (CAMP/Tibet) and Tibetan observation and research platform (TORP), the land surface emissivity (LSE) from the Moderate Resolution Imaging Spectroradiometer (MODIS), and the water vapor content from the National Centers for Environmental Prediction (NCEP) Climate Forecast System Reanalysis (CFSR) project. The 10-day composite hourly LST data were generated via the maximum value composite (MVC) method to reduce the cloud effects. The derived LST was validated by the field observations of CAMP/Tibet and TORP. The results show that the retrieved LST and in situ data have a very good correlation (with root mean square error (RMSE), mean bias (MB), mean absolute error (MAE) and correlation coefficient (R) values of 1.99 K, 0.83 K, 1.71 K, and 0.991, respectively). Together with other characteristic parameters derived from polar-orbiting satellites and meteorological forcing data, the energy balance budgets have been retrieved finally. The validation results showed there was a good consistency between estimation results and in-situ measurements over the TP, which prove the robustness of the proposed estimation

    Despite otological surgerical progress improving clinical congenital ossicular malformation management, some cases remain inadequately treated. We report 27 cases of congenital ossicular malformation, focusing on reasons for remaining or delayed postoperative hearing loss evaluated in 27 congenital ossicular malformation cases in Kyoto Prefecture from 2002 to 2008. Overall success was 93% (25/27) 6 months postoperatively. Two ears had no hearing improvement and three delayed hearing loss 8 to 48 months postoperatively. The first two ears underwent small fenestration stapedotomy with malleus attachment piston, and the other three tympanoplasty type III using an autologous ossicle or total ossicular replacement prosthesis (TORP) as a columella. We discuss problems and solutions using a malleus attachment piston or prosthesis, preoperative audio- and radiological findings, and operative findings including facial nerve anomaly and congenital cholesteatoma.     Ossiculoplasty is a surgical procedure that recreates sound transmission of the middle ear in conductive hearing loss. Various materials have been used for ossicular reconstruction, but the most ideal material for ossiculoplasty remains controversial. The purpose of this study was to introduce a novel method of autologous ossiculoplasty, bone-cartilage composite graft (BCCG) and to compare its surgical results with different types of ossiculoplastic prostheses. A retrospective study was performed in a tertiary referral centre. Data of 275 patients who received ossiculoplasty using the three different materials of BCCG, Polycel ® and titanium were analysed according to type of ossiculoplasty: partial or total ossicular replacement prosthesis (PORP or TORP). Hearing results, complication rates and clinical parameters including age, sex, past history, preoperative diagnosis and surgery type were compared among different groups. Ossiculoplasty with BCCG showed satisfactory hearing outcomes and the lowest complication rate among the three different materials. In particular, its extrusion rate was 0%. We propose that the BCCG technique is a useful alternative method for ossiculoplasty, with proper patient selection. © 2017 John Wiley & Sons Ltd.

    The use of Serious Games (SG) in the health domain is expanding. In the field of Neurodegenerative Diseases (ND) such as Alzheimer's Disease, SG are currently employed to provide alternative solutions for patients' treatment, stimulation, and rehabilitation. The design of SG for people with ND implies collaborations between professionals in ND and professionals in SG design. As the field is quite young, professionals specialized in both ND and SG are still rare, and recommendations for the design of SG for people with ND are still missing. This perspective paper aims to provide recommendations in terms of ergonomic choices for the design of SG aiming at stimulating people with ND, starting from the existing SG already tested in this population: "MINWii", "Kitchen and Cooking", and "X-Torp". We propose to rely on nine ergonomic criteria: eight ergonomic criteria inspired by works in the domain of office automation: Compatibility, Guidance, Workload, Adaptability, Consistency, Significance of codes, Explicit control and Error management; and one ergonomic criterion related to videogame: the game rules. Perspectives derived from this proposal are also discussed.

    Full Text Available We present the Waterfilling circuit selection method, which we designed in order to mitigate the risks of a successful end-to-end traffic correlation attack. Waterfilling proceeds by balancing the Tor network load as evenly as possible on endpoints of user paths. We simulate the use of Waterfilling thanks to the TorPS and Shadow tools. Applying several security metrics, we show that the adoption of Waterfilling considerably increases the number of nodes that an adversary needs to control in order to be able to mount a successful attack, while somewhat decreasing the minimum amount of bandwidth required to do so. Moreover, we evaluate Waterfilling in Shadow and show that it does not impact significantly the performance of the network. Furthermore, Waterfilling reduces the benefits that an attacker could obtain by hacking into a top bandwidth Tor relay, hence limiting the risks raised by such relays. Waterfilling does not require any major change in Tor, and can co-exist with the current circuit selection algorithm.

Douze génotypes de pois (Asgrow, Jumbo, Lincoln, Merveille de Kelvedon, Purser, Rajai Torpe, Snajor Kosep Korai, Wando, Rondo, génotype local, Major Kosep Korai et Surgevil ont été étudiés pour leur résistance aux maladies (Oïdium, mildiou, anthracnose, brunissement, jaunisse apicale et leurs performances agronomiques (matières fraîches: racines, partie aérienne, nombre de branches fructifères/plante, nombre de fleurs/plante, nombre de gousses/plante, nombre de grains/gousse et rendements en grains/plante. La culture a été faite dans un milieu contrôlé (serre plastique sur tourbe noire durant 5,5 mois (octobre à avril. Les résultats obtenus montrent que seul le génotype Purser est résistant à toutes les maladies et que le génotype Surgevil est sensible uniquement à la jaunisse apicale. Par contre, le génotype local est sensible aux trois maladies les plus fréquentes (Oïdium, Mildiou et Anthracnose. En ce qui concerne la croissance végétative, c'est le génotype Asgrow qui a synthétisé le plus de matière fraîche, deux fois plus que la matière fraîche synthétisée par le génotype Purser. Toutefois, le rendement élevé en matière fraîche ne contribue pas à un taux de nouaison élevé. En effet, seuls les génotypes ayant donné un rendement faible en matière fraîche (Snajor Kosep Korai, Asgrow, Major Kosep Korai, Rajai Torpe et Purser ont eu le taux de nouaison le plus élevé, supérieur à 30%. Chez ce groupe, le bon rendement le plus élevé (&gt; 9 g/plante résulte du nombre de gousses/ plante (7,5 à 21,6 et du nombre de grains/gousse (2,8 à 4,92. Por la ventana lateral del apartamento se cuela un zumbido espasmódico y ligero, lo producen los escasos automóviles que cruzan la avenida de cuando en cuando. El edificio que habito parece un Titán dormido, trasmite una molicie áspera y duradera, pero eso no quiebra la armonía del ambiente, por el contrario, le otorga sentido al clima, que es casi tibio. Son las cinco pasado el meridiano y los árboles juguetean con una brisa ligera y torpe, que de cuando en cuando choca con los cristales de las ventanas y se dispersa en fuga. Yo me acerco al balcón y observo las construcciones vecinas. Son moles grises, aunque su color sea blanco, verde o azul, parecen grises y dormitan igual que la antigua estructura desde donde fisgoneo. Vivo en una zona residencial de clase media, construida al interés de personas dispuestas a la caminata diurna y la larga y serena conversación vespertina. A doscientos metros de donde estoy hay una plaza, muchos árboles, una estación del Metro, varios locales comerciales y una gran avenida principal. Vivo en Altamira, baluarte de la reacción sociopolítica al gobierno del ex teniente coronel del ejército Hugo Chávez Frías.

Para ellos el materialista es un hombre sin sentimientos ni corazón, sin ideales de ninguna especie, sin ningún principio moral, ni norma alguna espiritual. Un hombre que ha llegado a destruir lo más noble y bello de la vida humana, el espíritu, para subyugar todo anhelo a la materia, a los sentidos y a la carne. Por eso le creen un sér torpe, vicioso, concupiscente y sensual. A ese grupo de estudiantes quiero aclararles el concepto, vulgarizándolo solamente, ya que una distinción profunda y plena de las dos únicas teorías que en definitiva luchan en el mundo, la idealista y la materialista, la encontrarán mejor expuesta en cualquier honrado compendio, de esos que se cubren de polvo y abandono en una mísera pena de arrabal. El materialismo es una escuela filosófica que nos enseña a concebir el mundo y a concebir el hombre de una manera más razonable que el idealismo, más de acuerdo con la realidad científica, .más en armonía con la verdad biológica, y por tanto, de un modo más humano y natural.

    Full Text Available O objetivo deste trabalho foi determinar a concentração de polifenóis totais, flavonóides, vitamina C e carotenóides totais em 61 acessos de banana pertencentes ao Banco Ativo de Germoplasma de bananeira da Embrapa Mandioca e Fruticultura. Foram detectadas diferenças significativas para todas as características analisadas por meio da análise de variância. A média para os teores de polifenóis totais entre os 61 acessos de banana foi de 38,06mg 100g-1, com variação de 12,51mg 100g-1 para o triplóide 'Torp' a 257,80mg 100g-1 para o tetraplóide 'Teparod'. A média para flavonóides foi de 2,09mg 100g-1, variando de 0,85mg 100g-1 ('Maravilha' AAAB a 6,63mg 100g-1 ('Teparod' ABBB. Para vitamina C, a média foi de 21,60mg 100g-1, variando de 8,60mg 100g-1 (tetraplóide 'Bucaneiro' a 76,82mg 100g-1 (tetraplóide 'Teparod'. A média do teor de carotenóides totais foi de 4,34mg g-1, variando de 1,18mg g-1 ('Champa Madras', triplóide ABB a 19,24mg g-1 ('Saney', triplóide AAB. Os diplóides Modok Gier e NBA-14 apresentaram, respectivamente, quatro e cinco vezes mais carotenóides totais que as cultivares do grupo Cavendish ('Nanica', 'Willians' e 'Lacatan'. Esses resultados permitem concluir sobre a possibilidade da obtenção de cultivares com altos níveis de compostos funcionais por meio de cruzamentos e seleção. Cultivares com este perfil têm potencial como alimento funcional, ajudando a prevenir doenças, por meio da neutralização de radicais livres.The objective of this research was to determine the concentration of polyphenols, flavonols, vitamin C and carotenoid and in 61 banana accessions from Musa germplasm collection from Embrapa Cassava and Fruits, Brazil.     This study was performed to evaluate the usefulness and limitations of three dimensional (3-D) imagings of stapes in the middle ear by high speed helical CT. One dissected human temporal bone, ten normal and diseased ears were scanned with a slice of 1.0 mm and reconstructed in a thickness of 0.2-0.5 mm. Every specimen of 3-D can be observed in any plane and from any direction. Ossicular imagings of the temporal bone in 3-D were reconstructed as if the malleus, incus and stapes were observed under microscope. The whole structure of stapes was impossible to be represented by two dimensional CT heretofore in use, but 3-D in our study showed the head, crus and foot plate of the stapes in detail. Stapedial imagings of 3-D CT in normal ears showed the same findings as those recorded in temporal bone. Preoperative diagnostic findings of ossicles in the affected ears were very useful. Especially in ossicular anomalies, 3-D CT was positive in diagnosis and its accuracies were confirmed with operative observation. For the postoperative evaluation concerning the ossicular reconstruction, i.e. TORP and PORP, 3-D CT was also important method. It could present an anatomical relation between those prosthesis and the oval window. High speed helical CT can scan an object more quickly and clearly than formerly used CT, and its biological damage for human is less than that of the others. 3-D CT can be more clearly reconstructed with helical CT than former CT.   10. Libraries as 'everyday' settings: the Glasgow MCISS project.

    A settings-based approach is now well-established in health promotion, initially undertaken in conventional places like schools and workplaces, but more recently being expressed in a wider range of what Torp et al. call 'everyday' settings. In this context, libraries have emerged as another potential setting whose ubiquity and accessibility suggests that they may be particularly effective in addressing health inequalities. Drawing on a case study-the Glasgow Macmillan Cancer Information and Support Services Library project-this paper reports on the potential for seeing 'libraries as settings' and in the context of a set of associated theoretical resources, specifically scrutinizes the nature of initiative implementation. Data were drawn from multiple sources: semi-structured interviews and focus groups with strategic partners and stakeholders, operational staff, project volunteers, service users and members of the general public. Qualitative data were complemented by quantitative insights from surveys with members of the partnership, libraries staff and volunteers. Despite some concerns associated with potentially hostile cultural and financial contexts that might threaten longer term sustainability, insights suggested that in pragmatic terms, the project was attracting sizable 'footfall' and successfully addressing a range of needs. Additionally, the formal implementation processes associated with project implementation were considered to have been highly successful in embedding the model into the library culture. In summary, there is evidence that libraries have the potential to be considered as supportive settings and could act as a model for an emergent vision of what libraries do. © The Author 2016.   11. Endoscopic middle ear exploration in pediatric patients with conductive hearing loss.

    To describe our indications, findings, and outcomes for transcanal endoscopic middle ear exploration in pediatric patients with conductive hearing loss of unknown etiology, without effusions. Prospective case series for all pediatric patients undergoing totally endoscopic transcanal middle ear exploration between April 2012 and October 2015 at a pediatric tertiary care referral hospital. Demographic data, operative findings, and hearing results were reviewed. 21 cases were performed in 20 ears (1 revision). Average age at surgery was 7.98 years and average follow up was 2.1 years. Middle ear pathology identified on CT imaging was confirmed in 55% of cases while identified in 45% of cases where pre-operative imaging was non-diagnostic. 6/20 patients (30%) had an ossicular deformity. 8/20(40%) had bony ossicular fixation. 5/20(25%) had ossicular discontinuity. 2/20(10%) had facial nerve dehiscence impinging on the stapes. 15% had adhesive myringosclerosis or severe granulation causing hearing loss. Prosthetic ossiculoplasty was done in 7/21 (33.3%) of the cases, with 1 TORP, 3 PORPs, and 3 IS joint replacements. Imaging was predictive of intra-operative findings in 13/20 cases (55%). Trainees assisted in 16/21(76%) of cases. The average improvement of PTA was 11.65 dB (range -10 to 36.25), and the average ABG improved 10.19 (range -11.25 to 28.75). There were no perioperative complications or adverse events. The endoscopic transcanal approach for middle ear exploration offers excellent visualization and is one of the best applications for the endoscopes in pediatric otology cases. This is particularly helpful for "unexplained" conductive hearing loss where ossicular deformity/fixation/discontinuity is suspected. The etiology of the conductive hearing loss was definitively found in 100% of cases, and can be repaired in the same sitting when applicable. Copyright © 2017 Elsevier B.V. All rights reserved.

  13. Endoscopy-Assisted Ear Surgery for Treatment of Chronic Otitis Media With Cholesteatoma, Adhesion, or Retraction Pockets.

    The objective of this study was to analyze the results of endoscopy-assisted ear surgery for the treatment of chronic otitis media with cholesteatoma, adhesion, or retraction pockets.Fifty-one patients who underwent oto-endoscopy-assisted canal wall up tympanomastoid surgery and/or limited anterior atticotomy with tympanoplasty for chronic otitis media with cholesteatoma, adhesion, or retraction pocket from 2006 to 2013 have been included in this study. Eradication of the disease from the middle ear and mastoid air cells was achieved by combination of the oto-microscobic and oto-endoscopic approaches. Second look surgery was performed 18 months later from the initial surgery in selected patients. Age, gender, pre-/postoperative otoscopy findings/audiograms, type of the used prostheses, and follow-up time were obtained from the patient's file. Anatomic integrity rates of the tympanic membrane, the mean gains of air bone gap, on pure-tone audiogram at 4 frequencies and existence of the residual disease were reviewed parameters. Functional evaluation was made in patients with intact tympanic membrane.Of the 51 patients, the ratios of the chronic otitis media with cholesteatoma and isolated adhesive otitis or retraction pocket cases were 74.5% (38/51) and 25.5% (13/51), respectively. Ossicular chain reconstruction was made with PORP in 27 patients and TORP in 20 patients, whereas the ossicular chain was intact in 4 patients. Anatomic integrity rates of the tympanic membrane were 90.2% (46/51). The overall (n = 46) pre-/postoperative mean ABG obtained at 4 frequencies were 28.3 ± 12.26 and 9.18 ± 5.68 dB (P otitis media with cholesteatoma patients. However, there was no recurrence or new cholesteatoma formation in isolated retraction pockets or adhesive otitis patients.Oto-endoscopic eradication of the cholesteatoma or epithelial tissue from hidden area after the all visible cholesteatoma removal by oto-microscope improves the quality of surgery

    Full Text Available Abstract: Slave-girls, and in particular singing slave-girls, hold a prominent place in Islamic literary sources. These sources provide quite a number of stories in which the masters of slave-girls fall deeply in love with them, and then, when faced with the prospect of separation or are indeed separated from them, humble themselves and risk losing their honour, all of their wealth, and even their own lives in order to be reunited with the girl whom they love. In some stories, intelligent and learned slave-girls take the initiative to preserve their relationships with their masters who are often depicted as inept and clueless. In the end, the girl is typically given her freedom and marries her master. Although the men are the legal masters of the slave-girls, it seems that there is an inversion of the master/slave roles in the tales and that it is the slave-girl who controls the destiny of both.Resumen: Las esclavas, con mención especial de las que cantan, tienen un lugar de preferencia en las fuentes literarias islamistas. Estas fuentes proveen un gran número de cuentos en los que los amos de estas esclavas se enamoran de ellas y luego al afrontarse la posibilidad de separación, y de hecho los dos se encuentran separados, se humillan y corren el riesgo de perder su honor, toda su riqueza y hasta sus propias vidas para reunirse con la chica que aman. En algunos cuentos, esclavas listas y doctas toman la iniciativa para preservar sus relaciones con los dueños, quienes parecen ser torpes y negados. A fin de cuentas, la chica suele recibir su libertad y se casa con el amo. A pesar de ser el amo legal de las esclavas, parece que hay una inversión de los papeles de amo/esclavo en estos cuentos y es la esclava la que controla el destino de los dos.

    Full Text Available Objetivou-se verificar qual o melhor estádio embrionário para o cultivo de embriões imaturos oriundos de frutos provenientes de hibridação entre 'Pêra Rio' x 'Poncã' , bem como o efeito de diferentes concentrações do meio de cultura MT. Os embriões em diferentes estádios de desenvolvimento (globulares, torpedo e cordiforme foram excisados e inoculados em tubos de ensaio contendo 15 mL do meio MT com diferentes concentrações (0; 50; 100 e 150% da composição original e acrescido de 50 g.L-1 de sacarose. Após a inoculação, os embriões foram incubados à 27±1ºC, fotoperíodo de 16 horas e irradiância de 32 mmol.m-2.s-1. Após 90 dias, avaliou-se o comprimento da parte aérea e do sistema radicular, massa fresca e número de folhas das plântulas. Melhor desenvolvimento dos embriões imaturos foi obtido em estádio cotiledonar e com a concentração de 150% do meio MT.Embryos obtained from Citrus sinensis Osbeck 'Pêra Rio' x Citrus reticulata Blanco 'Poncã' crossings were excised in diverse phases of development and cultivated in different medium concentrations. Embryos in globular, torped and cotiledonary stage were excised and inoculated individually in test tubes with 15 mL in distinct MT medium concentrations (0; 50; 100 and 150% of original composition and added sucrose (50 g.L-1. After inoculation, the embryos were maintained in growth room at 27±1ºC, 16 hour photoperiod and 32 umol.m-2.s-1 irradiance. After 90 days, the length of the aerial part and roots system, fresh mass and number of leaves, were evaluated. The best results were obtained by incubating embryos in the cotiledonary stage, in MT medium with concentrations of 150%.

    Full Text Available Objetivou-se estudar o efeito da cinetina, GA3 e ANA na indução in vitro de embriões somáticos de cafeeiro pela via direta. Segmentos foliares retirados de plântulas cultivadas in vitro foram inoculados em meio de cultura 'MS' com 50% dos sais contendo as seguintes combinações de cinetina (0; 1; 2; 4 e 8 mg L-1 e GA3 (0; 2,5; 5; 10 e 20 mg L-1. Os meios de cultura utilizados tiveram pH ajustado para 5,8 ± 1 antes de serem autoclavados. O experimento foi mantido em sala de crescimento a 25 ± 1ºC. Avaliou-se número total de embriões somáticos, o número de embriões cotiledonares, o número de embriões torpedo e a média dos comprimentos dos embriões. A ação combinada entre cinetina, GA3 e ANA estimulou a indução de embriões somáticos pela via direta. O maior comprimento de embriões foi observado quando se utilizou 8 mg L-1 de cinetina e 8,0 mg L-1 de ANA ou 17 mg L-1 de GA3 e 8,0 mg L-1 de ANA isoladamente.It was aimed to study kinetin, GA3 and ANA effects in the in vitro induction of direct somatics embryos. Leaf segments withdrawn from plantlets in vitro were inoculated in 'MS'50% containing the following combination of kinetin (0; 1; 2; 4 and 8 mg L-1 and GA3 (0; 2,5; 5; 10 and 20 mg L-1. The culture media utilized had their pH adjusted to 5,8 ± 1 before being autoclaved. The experiment was carried out growth room at 25 ± 1ºC. Total number of embryos, number of embryos cotiledonar, number of embryos of torped and length of embryo were evaluated. The combination between kinetin and GA3 promoved induction of embryos. The use of kinetin 8 mg L-1 and GA3 17 mg L-1 not associate in medium with 8,0mg L-1 of ANA, promoter higher rates in vitro.

    Full Text Available This article focuses on L’Alcorano di Macometto, written by Giovanni Battista Castrodardo from Belluno (1588 ca.-1517 ca., and published by Andrea Arrivabene at Venice in 1547. Although this text is considered to be a paraphrase of the Latin translation of the Qur’ān made by Robert of Ketton (1143 AD, a closer look shows that it was a handy pocket encyclopedia of the history of Islam and the Ottoman empire. Following a suggestion of Alessandro D’Ancona (1889, I analyze the historical texts published in the introduction to the Alcorano and especially Castrodardo’s biography of the prophet Muḥammad. This biography includes a prayer addressed to Muḥammad through the character of the monk Sergio-Bahīrā in order to persuade him to dethrone Emperor Heraclius. A deep linguistic analysis of this speech sheds light on Castrodardo’s reading of Machiavelli. While this new portrayal of Muhammad is based on medieval sources, it presents the Prophet of Islam as a distinctly Renaissance Prince, a lawgiver and a “Prophet Armed”. It also reflects the pro-Ottoman and anti-Imperial hopes of Italian circles linked to the French embassy in Venice.Este artículo se centra en L’Alcorano di Macometto, escrito por Giovanni Battista Castrodardo de Belluno (1517 ca.-1588 ca. e impreso por Andrea Arrivabene en Venecia en 1547.     Full Text Available It has been thought that Soviet society was characterised by a phenomenon that only the truth sanctioned by the authorities was talked about in public, even in cases when the real, diametrically opposite truth was actually known. Such dualism and duplicity were also characteristics of Soviet journalism. For example, Peeter Vihalemm and Marju Lauristin (2004 have drawn attention to ambivalence as one of the characteristic features of Soviet Estonian journalism: on the one hand, it was a part of the totalitarian ideology and propaganda system, but on the other hand, it was a bearer of the cultural sphere, cultural identity and opposition. However, according to the Marxist-Leninist canons, journalism was only a means for achieving the political and ideological goals of the Communist Party. Therefore, from the time it was founded, Eesti Televisioon (ETV (Estonian Television Company was highly controlled by the party. ETV was granted the status of an ‘ideology institution’ and the party treated TV journalists as ideology workers. Consequently, the requirements for them were high, e.g., they had to have the ability to “evaluate social phenomena from Marxist-Leninist viewpoints” and the skill to defend this ideology and to expose hostile ideologies. Hagi Shein, having researched the history of ETV (2004, has argued that journalists could express themselves more freely on apolitical areas of TV programmes, and although self-censorship was strong, they learned to make use of different situations, to balance on the edge between the allowed and the prohibited, to deceive the censors, and to use different contexts. The author of this article researched the minutes and records of the party meetings of the Teleradio Committee of the Estonian Soviet Socialist Republic and ETV to find the double standards of Communists working at ETV. Mientras tanto, en similar sentido conflictual, la campaña teórica de Samuel Huntington por todo el planeta, responde con un paradigma postguerra fría capaz de inflamar el conflicto internacional en un sentido no menos beligerante con anti-Occidente que el empleado por los imanes radicales. Como ha señalado Amartya Sen, al priorizar de esta forma la identidad religiosa, la respuesta de Occidente al terrorismo internacional como «terrorismo islámico» es muy torpe pues se magnifica la importancia de las autoridades religiosas en detrimento de los cauces gubernativos en la resolución de los problemas.

  4. Using Biosurfactants Produced from Agriculture Process Waste Streams to Improve Oil Recovery in Fractured Carbonate Reservoirs

    This report describes the progress of our research during the first 30 months (10/01/2004 to 03/31/2007) of the original three-year project cycle. The project was terminated early due to DOE budget cuts. This was a joint project between the Tertiary Oil Recovery Project (TORP) at the University of Kansas and the Idaho National Laboratory (INL). The objective was to evaluate the use of low-cost biosurfactants produced from agriculture process waste streams to improve oil recovery in fractured carbonate reservoirs through wettability mediation. Biosurfactant for this project was produced using Bacillus subtilis 21332 and purified potato starch as the growth medium. The INL team produced the biosurfactant and characterized it as surfactin. INL supplied surfactin as required for the tests at KU as well as providing other microbiological services. Interfacial tension (IFT) between Soltrol 130 and both potential benchmark chemical surfactants and crude surfactin was measured over a range of concentrations. The performance of the crude surfactin preparation in reducing IFT was greater than any of the synthetic compounds throughout the concentration range studied but at low concentrations, sodium laureth sulfate (SLS) was closest to the surfactin, and was used as the benchmark in subsequent studies. Core characterization was carried out using both traditional flooding techniques to find porosity and permeability; and NMR/MRI to image cores and identify pore architecture and degree of heterogeneity. A cleaning regime was identified and developed to remove organic materials from cores and crushed carbonate rock. This allowed cores to be fully characterized and returned to a reproducible wettability state when coupled with a crude-oil aging regime. Rapid wettability assessments for crushed matrix material were developed, and used to inform slower Amott wettability tests.     understand water-ice-air-ecosystem interactions over heterogeneous landscape of the Tibetan Plateau is to establish the comprehensive research stations over the different land surfaces of the Tibetan Plateau. With support from various agencies in the People' s Republic of China, the Tibetan Observation and Research Platform (TORP) is now implementing. Firstly, 5 comprehensive observation and research stations established by the Institute of Tibetan Plateau Research, Chinese Academy of Sciences, are introduced. Then the preliminary observational results are shown in the paper. Finally, the difficulty in observation and study of water-ice-air-ecosystem interactions of the Tibetan Plateau and the possible solutions are presented.

    Modulation of the diurnal variations in the convective activities associated with day-by-day changes of surface flux and soil moisture was observed in the beginning of the monsoon season on the central Tibetan plateau (Sugimoto et al., 2008) which indicates the importance of land-atmosphere interactions in determining convective activities over the Tibetan plateau. Detailed interaction processes need to be studied by experiments designed to evaluate a set of hypotheses on mechanisms and linkages of these interactions. A possible function of vegetation to increase precipitation in cases of Tibetan High type was suggested by Yamada and Uyeda (2006). Use of satellite derived plateau scale soil moisture (Wen et al., 2003) enables the verification of these hypotheses (e.g. Trier et al. 2004). To evaluate these feedbacks, the mesoscale WRF model will be used because several numerical experiments are being conducted to improve the soil physical parameterization in the Noah land surface scheme in WRF so that the extreme conditions on the Tibetan plateau could be adequately represented (Van der Velde et al., 2009) such that the impacts on the structure of the atmospheric boundary layer can be assessed and improved. The Tibetan Observational Research Platform (TORP) operated by the Institute of Tibetan Plateau (Ma et al., 2008) will be fully utilized to study the characteristics of the plateau climate and different aspects of the WRF model will be evaluated using this extensive observation platform (e.g. Su et al., 2012). Recently, advanced studies on energy budget have been done by combining field and satellite measurements over the Tibetan Plateau (e.g. Ma et al., 2005). Such studies, however, were based on a single satellite observation and for a few days over an annual cycle, which are insufficient to reveal the relation between the land surface energy budget and the Asian monsoon over the Tibetan plateau. Presently these species do not occur at latitudes higher than the Mediterranean or the Atlantic Moroccan coast.After the formation of the fossil deposits of Gruta da Furninha, the palaeoclimatic evolution is not known until ca. 45 Ka calBP, represented by the palaeontological sites of Vale de Janela in the Estremadura littoral and São Torpes in the Alentejo littoral. In both sites there were found species related to a cool and wet temperate climate, but Myrica also occurs, a termophilic genus. A climate cooler than the Mediterranean is in agreement with the presence of the mountain goat in the upper Mousterian levels of gruta da Oliveira dating before 43/42 Ka calBP. Afterwards, the climate became progressively warmer and Mediterranean-type: the mountain goat disappeared from Gruta da Oliveira and Mediterranean rodents are present in the Level 8 of that cave, dating from 38/37 Ka calBP, while Cepaea nemoralis appeared in Lapa dos Furos, dating from 40 Ka calBP.From 36 Ka calBP on, there was a climatic cooling and the mountain goat reappears in low altitude mountain ranges (Gruta Nova da Columbeira and Gruta da Figueira Brava. In fact, the weather conditions were probably cooler than in the present and comparable to those in the cantabrian region, as suggested by the findings in Gruta da Figueira Brava. But the presence of the land turtle, which was abundant in Gruta Nova da Columbeira (up to 34-31 Ka calBP demands summer temperatures ca. 20-30 ºC for egg hatching. On the other hand, the microfauna from Level K in Gruta do Caldeirão, including Allocricetus bursae, shows how steppic conditions migrated to the western part of the Iberian Peninsula and prevailed when the first industries of the Upper Paleolithic occurred, an evolved phase of the Aurignacian, about probably 35-34 Ka calBP.Within this particular paleoclimatic framework of the western and southwestern parts of the Peninsula, it is possible
