    Full Text Available Nanotecnology products have been used in wide applications in chemistry, electronics, energy generation, and medicine. Despite significant interest in developing quantum dots (QDs for biomedical applications, many researchers are convinced that QDs will never be used for the treatment of patients because of their potential toxicity. In various in vitro cell culture studies, the cytotoxic properties of some QD have been demonstrated and they have been suggested to be toxic in humans. In this study, the cytotoxic properties of Ag2S-(Meso-2,3-Dimercapto Succinic acid nanomaterials in V79 cells (Chinese lung fibroblast cell line were determined by MTT assay. The genotoxic effects of Ag2S-(Meso-2,3-Dimercapto Succinic acid were evaluated by the alkaline single cell gel electrophoresis. The cells were treated with Ag2S-(Meso-2,3-Dimercapto Succinic acid at the concentrations of 5- 2000 µg/ml. No cytotoxic effect of Ag2S-(Meso-2,3-Dimercapto Succinic acid at all concentrations studied was observed. No significant increases in DNA damage were found at the studied concentrations when compared to negative control in V79 cells. In conclusion, further in vitro and in vivo studies are required to determine the safety doses of Ag2S-(Meso-2,3-Dimercapto Succinic acid.

    Disulfide bond formation is the decisive event in the protein folding to determine the conformation and stability of protein. To achieve this disulfide bond formation in vitro, we took 2,5-dimercapto-1,3,4-thiadiazole (DMcT) as a model compound. We found that disulfide bond formation takes place between two sulfhydryl groups of DMcT molecules in methanol.   3. 99mtechnetium-dimercapto-succinic acid renal scanning and excretory urography in diagnosis of renal scars in children

    We compared the ability of excretory urography (without tomography) and 99mtechnetium-dimercapto-succinic acid renal scanning to detect renal scars in 32 children with primary vesicoureteral reflux. These children did not have hydronephrosis, renal failure or urinary tract obstruction. In all cases both studies were conducted within a 10-month period. The findings from both modalities were in agreement for 51 of the 64 renal units evaluated (80%). Evaluation of the excretory urogram indicated 6 cases of diffuse and 2 of focal scarring that were not detected by evaluation of the renal scan. The sensitivity of excretory urography to detect renal scars was 84% and the specificity was 83%. The 99mtechnetium-dimercapto-succinic acid renal scan showed 5 cases of focal renal scarring not detected by excretory urography. The sensitivity of the renal scan to detect renal scars was 77% and the specificity was 75%. We conclude that neither study alone could effectively replace the other for the detection of renal scars, and recommend that both be included in the initial evaluation and followup of patients with renal scars

  4. Importance of methodology on (99m)technetium dimercapto-succinic acid scintigraphic image quality: imaging pilot study for RIVUR (Randomized Intervention for Children With Vesicoureteral Reflux) multicenter investigation.

    We reviewed our experience with (99m)technetium dimercapto-succinic acid scintigraphy obtained during an imaging pilot study for a multicenter investigation (Randomized Intervention for Children With Vesicoureteral Reflux) of the effectiveness of daily antimicrobial prophylaxis for preventing recurrent urinary tract infection and renal scarring. We analyzed imaging methodology and its relation to diagnostic image quality. (99m)Technetium dimercapto-succinic acid imaging guidelines were provided to participating sites. High-resolution planar imaging with parallel hole or pinhole collimation was required. Two core reviewers evaluated all submitted images. Analysis included appropriate views, presence or lack of patient motion, adequate magnification, sufficient counts and diagnostic image quality. Inter-reader agreement was evaluated. We evaluated 70, (99m)technetium dimercapto-succinic acid studies from 14 institutions. Variability was noted in methodology and image quality. Correlation (r value) between dose administered and patient age was 0.780. For parallel hole collimator imaging good correlation was noted between activity administered and counts (r = 0.800). For pinhole imaging the correlation was poor (r = 0.110). A total of 10 studies (17%) were rejected for quality issues of motion, kidney overlap, inadequate magnification, inadequate counts and poor quality images. The submitting institution was informed and provided with recommendations for improving quality, and resubmission of another study was required. Only 4 studies (6%) were judged differently by the 2 reviewers, and the differences were minor. Methodology and image quality for (99m)technetium dimercapto-succinic acid scintigraphy varied more than expected between institutions.   5. Highly Productive Synthesis, Characterization, and Fluorescence and Heavy Metal Ion Adsorption Properties of Poly(2,5-dimercapto-1,3,4-thiadiazole Nanosheets

    Full Text Available Poly(2,5-dimercapto-1,3,4-thiadiazole (PBT nanosheets were synthesized by chemical oxidative synthesis under mild conditions. The media, oxidant species, monomer concentrations, oxidant/monomer molar ratio, and temperature were optimized to achieve higher yields and better performance. The molecular structure, morphology, and properties of the nanosheets were analyzed by Fourier transform infrared (FT-IR, ultraviolet-visible (UV-Vis, and fluorescence spectroscopies, wide-angle X-ray diffraction (WAXD, matrix-assisted laser desorption/ionization/time-of-flight (MALDI-TOF mass spectrometry, X-ray photoelectron spectroscopy (XPS, scanning electronic microscopy (SEM, transmission electron microscopy (TEM, and simultaneous thermogravimetry and differential scanning calorimetry (TG-DSC. It was found that the polymerization of 2,5-dimercapto-1,3,4-thiadiazole occurs via dehydrogenation coupling between two mercapto groups to form the –S–S– bond. PBTs show the highest polymerization yield of up to 98.47% and form uniform nanosheets with a thickness of 89~367 nm. poly(2,5-dimercapto-1,3,4-thiadiazole polymers (PBTs exhibit good chemical resistance, high thermostability, interesting blue-light emitting fluorescence, and wonderful heavy metal ion adsorption properties. Particularly, the PBT nanosheets having a unique synergic combination of three kinds of active –S–, –SH, and =N– groups with a moderate specific area of 15.85 m2 g−1 exhibit an ultra-rapid initial adsorption rate of 10,653 mg g−1 h−1 and an ultrahigh adsorption capacity of up to 680.01 mg g−1 for mercury ion, becoming ultrafast chelate nanosorbents with a high adsorption capacity. With these impressive properties, PBT nanosheets are very promising materials in the fields of water treatment, sensors, and electrodes.

    Lewisite is a potent chemical warfare arsenical vesicant that can cause severe skin lesions. Today, lewisite exposure remains possible during demilitarization of old ammunitions and as a result of deliberate use. Although its cutaneous toxicity is not fully elucidated, a specific antidote exists, the British anti-lewisite (BAL, dimercaprol) but it is not without untoward effects. Analogs of BAL, less toxic, have been developed such as meso-2,3-dimercaptosuccinic acid (DMSA) and have been employed for the treatment of heavy metal poisoning. However, efficacy of DMSA against lewisite-induced skin lesions remains to be determined in comparison with BAL. We have thus evaluated in this study the therapeutic efficacy of BAL and DMSA in two administration modes against skin lesions induced by lewisite vapor on SKH-1 hairless mice. Our data demonstrate a strong protective efficacy of topical application of dimercapto-chelating agents in contrast to a subcutaneous administration 1 h after lewisite exposure, with attenuation of wound size, necrosis and impairment of skin barrier function. The histological evaluation also confirms the efficacy of topical application by showing that treatments were effective in reversing lewisite-induced neutrophil infiltration. This protective effect was associated with an epidermal hyperplasia. However, for all the parameters studied, BAL was more effective than DMSA in reducing lewisite-induced skin injury. Together, these findings support the use of a topical form of dimercaprol-chelating agent against lewisite-induced skin lesion within the first hour after exposure to increase the therapeutic management and that BAL, despite its side-effects, should not be abandoned. - Highlights: • Topically applied dimercapto-chelating agents reduce lewisite-induced skin damage. • One topical application of BAL or DMSA is sufficient to reverse lewisite effects. • Topical BAL is more effective than DMSA to counteract lewisite-induced skin damage.

  7. Poly(2,5-dimercapto-1,3,4-thiadiazole) as a Cathode for Rechargeable Lithium Batteries with Dramatically Improved Performance

    Organosulfur compounds with multiple thiol groups are promising for high gravimetric energy density electrochemical energy storage. We have synthesized a poly(2,5-dimercapto-1,3,4-thiadiazole) (PDMcT)/poly(3,4-ethylenedioxythiophene) (PEDOT) composite cathode for lithium-ion batteries with a new method and investigated its electrochemical behavior by charge/discharge cycles and cyclic voltammetry (CV) in an ether-based electrolyte. Based on a comparison of the electrochemical performance with a carbonate-based electrolyte, we found a much higher discharge capacity, but also a very attractive cycling performance of PDMcT by using a tetra(ethylene glycol) dimethyl ether (TEGDME)-based electrolyte. The first discharge capacity of the as-synthesized PDMcT/PEDOT composite approached 210 mAh g -1 in the TEGDME-based electrolyte. CV results clearly show that the redox reactions of PDMcT are highly reversible in this TEGDME-based electrolyte. The reversible capacity remained around 120 mAh g -1 after 20 charge/discharge cycles. With improved cycling performance and very low cost, PDMcT could become a very promising cathode material when combined with a TEGDME-based electrolyte. The poor capacity in the carbonate-based electrolyte is a consequence of the irreversible reaction of the DMcT monomer and dimer with the solvent, emphasizing the importance of electrolyte chemistry when studying molecular-based battery materials. The nature of the electrolyte has a dramatic effect on the performance of poly(2,5-dimercapto-1,3,4-thiadiazole) (PDMcT) as a cathode material in lithium-ion batteries. Whereas the use of an ethylene/diethyl carbonate (EC/DEC)-based electrolyte resulted in very poor performance, the use of a tetra(ethylene glycol) dimethyl ether (TEGDME)-based electrolyte dramatically improved the performance in terms of both the discharge capacity and capacity retention (see scheme).     Full Text Available Avaliou-se a função renal em 10 pacientes com leishmaniose muco-cutânea tratados com glucantime (antimoniato de Meglumine, Rhodia ou Pentostam (estibogluconato de sólio, Wellcome. Durante o uso das drogas, verificou-se a existência de um defeito na capacidade concentrante do rim, obtendo-se menores valores da osmolaridade urinária máxima e de depuração negativa máxima de água livre, neste período, em relação aos testes efetuados antes do tratamento. A capacidade de concentração urinária normalizou-se em 5, de 8 pacientes estudados no período de 15 a 30 dias, após a suspensão dos medicamentos, embora com valores de osmolaridade urinária máxima inferiores aos obtidos antes do tratamento. Em dois pacientes surgiu proteínúria, acima de 150 mg/dia, com o uso dos antimoniais, normalizando-se posteriormente. A depuração de creatinina endógena não se alterou significativamente com o uso das drogas. Os resultados sugerem que os antimoniais pentavalentes podem levar a uma disfunção tubular renal, caracterizada por um defeito na capacidade de concentrar a urina, reversível após a retirada dos medicamentos.

    protective/seropositive titers for Hib, HB, and tetanus and about 50% for diphtheria and Bordetella pertussis. At 15 months of age, virtually all the toddlers responded with a strong boost response to all the vaccine antigens, whether they received the DTPw-HB/Hib pentavalent vaccine or the DTPw/Hib vaccine as a booster. Both booster regimens were equally well tolerated, indicating that up to five doses of the HB vaccine can be given without impact on safety. Conclusions. Our study confirms that the DTPw-HB/Hib pentavalent vaccine is highly immunogenic as a primary vaccination in children who received an HB vaccine at birth, with the pentavalent combination inducing both persisting immunity and boostable memory. The pentavalent vaccine was safe both for primary and booster vaccinations. Thus, this study in Costa Rican infants supports the routine use of the pentavalent DTPw-HB/Hib vaccine as part of childhood vaccination programs in Latin America and the Caribbean.Objetivos. La vacuna combinada pentavalente DTPw-HB/Hib ha sido creada siguiendo la recomendación de la Organización Mundial de la Salud de añadir las vacunas contra la hepatitis b (HB y el Haemophilus influenzae tipo b (Hib a los programas de vacunación infantil.     Full Text Available FUNDAMENTO: A prevalência das doenças cardiovasculares (DCV tem aumentado nos últimos anos. A literatura revela que cerca de 35% dos eventos ateroscleróticos ocorrem na ausência dos fatores de risco clássicos, requerendo uma avaliação individual minuciosa para melhor caracterizar o risco. Os índices lipídicos tetravalente (LTI e pentavalente (LPI constituem uma nova e eficiente forma de avaliação do perfil lipídico e do risco para DCV. OBJETIVO: O presente estudo avaliou o LTI e o LPI em estudantes de graduação, estabelecendo estes índices em indivíduos saudáveis e correlacionando-os com o perfil lipídico tradicional. MÉTODOS: Participaram do estudo 110 estudantes, 48 (44% do sexo masculino e 62 (56% do sexo feminino, com média de idade de 20,9 ± 1,7 anos. Apolipoproteína-AI, apolipoproteína B, colesterol total, lipoproteína (a, triglicérides, HDL e LDL foram analisados usando-se métodos diagnósticos específicos. LTI e LPI foram calculados por meio das equações LTI = [colesterol total x triglicérides x lipoproteína (a / HDL] e LPI = [colesterol total x triglicérides x lipoproteína (a x apolipoproteína B / apolipoproteína A-I], respectivamente. RESULTADOS: Os valores de LTI e de LPI foram significativamente maiores nas mulheres quando comparados aos dos homens. Para os outros parâmetros, houve diferenças significativas entre os gêneros apenas para colesterol total, HDL e apolipoproteína A-I. Houve correlações positivas e significativas entre LDL e LTI e entre LDL e LPI. CONCLUSÃO: Os achados indicam que LTI e LPI estavam associados com LDL, um parâmetro não utilizado para calcular os índices lipídicos e amplamente usado na prática clínica para investigação do risco cardiovascular.

    Graphical abstract: Display Omitted -- Abstract: Organosulfur Compounds (OSCs) represent an attractive alternative as organic cathode materials for electrochemical energy storage (EES) applications. They intrinsically have high gravimetric capacity (although low volumetric) and are typically inexpensive, since they are composed of abundant elements (i.e. C, N, O, and S). However, OSCs, specifically thiolate-containing OSCs generally suffer from slow charge transfer kinetics. To mitigate the charge transfer limitations, conducting polymers (CPs) such as poly-3,4-ethylenedioxythiophene (PEDOT) have been employed as electrocatalysts. In this manuscript, we have covalently modified a PEDOT film with an OSC (i.e. 2,5-dimercapto-1,3,4-thiadiazole di-lithium salt (Li 2 DMcT)). We have developed a synthetic strategy that employs, for the first time, a post-polymerization modification reaction as a tractable and viable technique to modify organic materials for EES electrodes. Electrochemical characterization, via cyclic voltammetry showed the expected pseudocapacitive response of PEDOT with the superimposed faradaic processes of the covalently bound DMcT. Moreover, spectroscopic characterization using Raman spectroscopy provided mechanistic insights into the electrochemical reactions. Furthermore, we electropolymerized films onto coin-cell electrodes and tested them in half-cell configurations and found that the capacity retention of the films was significantly enhanced, when compared with the PEDOT/DMcT composites (mixed but not covalently bound)

    Full Text Available Foi avaliada a função renal de 11 pacientes com leishmaniose cutâneo-mucosa tratados com antimonial pentavalente na dose de 40mg SbV/kg/dia aplicada de 12/12 horas, em esquema contínuo, durante trinta dias. No estudo, um paciente apresentou insuficiência renal reversível e dois desenvolveram alterações enzimáticas hepáticas e eletrocardiográficas sendo o esquema terapêutico interrompido. Nos demais pacientes observou-se efeitos nefrotóxicos tais como diminuição da taxa de filtração glomerular, diminuição da capacidade de concentração urinária, avaliada por um jejum hídrico de 16 horas e aumento na fração de excreção de sódio. No exame do sedimento urinário observou-se um aumento no número de leucócitos e cilindros. Os resultados encontrados neste estudo sugerem que o tratamento com antimonial pentavalente na dose de 40mg SbV/kg/dia foi menos tolerado em virtude de seus efeitos tóxicos, não parecendo apresentar índice de cura superior ao esquema atualmente preconizado de 20mg SbV/kg/dia.The renal function of eleven patients with mucocutaneous leishmaniasis was analyzed in a prospective study realized at the School Hospital of University of Brasília. The patients were treated with doses of 40mg/kg/day of pentavalent antimony (SbV, in a continuous scheme during thirty days. In this study three patients were excluded, one patient with reversible renal failure and two patients with hepatic and cardiac malfunctions. In the other eight patients, severe nephrotoxics effects were observed, like reduction of glomerular filtration rate, reduction of the urinary concentration capacity, evaluated by a sixteen hours hydric fasting and an increase of sodium fractional excretion. An increase in the number of leucocytes and cylinders were observed at the urinary sediment exam. Finally, the results shows that the treatment with pentavalent antimony in doses of 40mg Sb/kg/day was less tolerated on account of its renal toxics

    Full Text Available Los antimoniales pentavalentes Glucantime® y Pentostam® son los medicamentos de primera línea usados en el tratamiento anti-Leishmania; sin embargo, no hay estudios in vivo que comparen su eficacia y toxicidad controlando variables del hospedero. Para evaluar la eficacia clínica se infectaron hámsteres en la pata trasera derecha con 106 promastigotes de Leishmania panamensis silvestre y transfectada con el gen de la luciferasa. Para evaluar la eficacia parasitológica se estandarizó la cuantificación de parásitos en los tejidos por luminometría. Tres semanas después de la inoculación, los animales se trataron intramuscularmente con Glucantime® o Pentostam® (30, 60 o 120 mg de SbV/kg por día por 20 días. La toxicidad se evaluó en hámsteres tratados intramuscularmente con 120, 160 o 240 mg de SbV/kg por día por 20 días de Glucantime® o Pentostam®. Los mecanismos de resistencia al antimonio pentavalente conocidos hasta el momento,se han descrito ampliamente en cepas del subgénero Leishmania, pero poco se sabe sobre lasproteínas involucradas en los mecanismos de resistencia presentes en cepas del subgénero Viannia,como Leishmania panamensis. Objetivo. Identificar proteínas diferencialmente expresadas entre las cepas de L. panamensis(UA140, sensible y resistente al antimonio pentavalente, y analizar el posible papel de estas proteínasen mecanismos de resistencia. Materiales y métodos. Las proteínas de las cepas, sensible y resistente al antimonio pentavalente, secompararon usando electroforesis bidimensional. Las proteínas con aumento de la expresión fueronaisladas e identificadas por espectrometría de masas mediante MALDI-TOF/TOF (Matrix AssistedLaser Desorption Ionization/Time of Flight. La expresión del ARNm de cinco de estas proteínas secuantificó mediante PCR en tiempo real. Resultados. Los geles bidimensionales de las cepas sensible y resistente detectaron 532±39 y 541±43manchas proteicas. Se encontraron 10 manchas con aumento de la expresión en la cepa resistente,identificadas como proteínas de choque térmico (Hsp60 mitocondrial, Hsp70 mitocondrial y citosólica,isomerasa de disulfuro, proteasa de cisteína, enolasa, factor de elongación 5-α, la subunidad 5-α delproteasoma y dos proteínas hipotéticas nombradas como Sp(2 y Sp(25. Conclusión.     The present invention relates to improved methods using an organic iron chelate to remove pollutants from fluids, such as flue gas. Specifically, the present invention relates to a process to remove NO[sub x] and optionally SO[sub 2] from a fluid using a metal ion (Fe[sup 2+]) chelate wherein the ligand is a dimercapto compound wherein the --SH groups are attached to adjacent carbon atoms (HS--C--C--SH) or (SH--C--CCSH) and contain a polar functional group so that the ligand of DMC chelate is water soluble. Alternatively, the DMC is covalently attached to a water insoluble substrate such as a polymer or resin, e.g., polystyrene. The chelate is regenerated using electroreduction or a chemical additive. The dimercapto compound bonded to a water insoluble substrate is also useful to lower the concentration or remove hazardous metal ions from an aqueous solution. 26 figures.

    Full Text Available FUNDAMENTO: A prevalência das doenças cardiovasculares (DCV tem aumentado nos últimos anos. A literatura revela que cerca de 35% dos eventos ateroscleróticos ocorrem na ausência dos fatores de risco clássicos, requerendo uma avaliação individual minuciosa para melhor caracterizar o risco. Os índices lipídicos tetravalente (LTI e pentavalente (LPI constituem uma nova e eficiente forma de avaliação do perfil lipídico e do risco para DCV. OBJETIVO: O presente estudo avaliou o LTI e o LPI em estudantes de graduação, estabelecendo estes índices em indivíduos saudáveis e correlacionando-os com o perfil lipídico tradicional. MÉTODOS: Participaram do estudo 110 estudantes, 48 (44% do sexo masculino e 62 (56% do sexo feminino, com média de idade de 20,9 ± 1,7 anos. Apolipoproteína-AI, apolipoproteína B, colesterol total, lipoproteína (a, triglicérides, HDL e LDL foram analisados usando-se métodos diagnósticos específicos. LTI e LPI foram calculados por meio das equações LTI = [colesterol total x triglicérides x lipoproteína (a / HDL] e LPI = [colesterol total x triglicérides x lipoproteína (a x apolipoproteína B / apolipoproteína A-I], respectivamente. RESULTADOS: Os valores de LTI e de LPI foram significativamente maiores nas mulheres quando comparados aos dos homens. Para os outros parâmetros, houve diferenças significativas entre os gêneros apenas para colesterol total, HDL e apolipoproteína A-I. Houve correlações positivas e significativas entre LDL e LTI e entre LDL e LPI.     The simple and easy-to-prepare bifunctional molecule 2,5-dimercapto-1,4-benzenedicarboxylic acid (H 4 DMBD) interacts with the increasingly harder metal ions of Cu + , Pb 2+ and Eu 3+ to form the coordination networks of Cu 6 (DMBD) 3 (en) 4 (Hen) 6 (1), Pb 2 (DMBD)(en) 2 (2) and Eu 2 (H 2 DMBD) 3 (DEF) 4 (3), where the carboxyl and thiol groups bind with distinct preference to the hard and soft metal ions, respectively. Notably, 1 features uncoordinated carboxylate groups and Cu 3 cluster units integrated via the thiolate groups into an extended network with significant interaction between the metal centers and the organic molecules; 2 features a 2D coordination net based on the mercapto and carboxylic groups all bonded to the Pb 2+ ions; 3 features free-standing thiol groups inside the channels of a metal-carboxylate-based network.     A method of diagnosing and treating occupational mercury poisoning is reviewed in this paper. The treating measures for the chelating syndrome are also described. Based on his exposure history, urinary mercury, and clinical symptom, the employee who exposed occupationally to mercury can be diagnosed to a certain degree according to the related diagnostic standards. The chelating agent, such as meso-2, 3-dimercapto-succinicacid (DMSA), is used for mercury-lustrating treatment. By reducing the chelating agent dose and supplying necessary trace element, the side effect of chelating treatment is decreased. (authors)

    Full Text Available Al plomo, metal pesado grisáceo y ubicuo, se le ha encontrado en lugares tan inverosímiles como los hielos fósiles de Groenlandia. Egipcios y hebreos lo usaron. Los fenicios c. 2000 AC trabajaron menas de plomo en España. Al final del s XX, en países desarrollados, la intoxicación por plomo pasó de problema ocupacional a problema de salud pública. Entre nosotros aún es frecuente la intoxicación ocupacional. El diagnóstico pasa por reconocer la existencia de plomo en el ambiente laboral y en tomar una buena historia clínica y ocupacional. El diagnóstico diferencial considera cuadros dolorosos neurológicos y abdominales. Plomo-sanguíneo y zinc-protoporfirina son análisis cruciales y fidedignos. En expuestos, el plomo-sanguíneo suele llegar a 40 ug/100 mL, aunque en trabajadores de industrias insalubres puede alcanzar 80 ug/100 mL y la zinc-protoporfirina ser mayor de 4 ug/g de hemoglobina. El tratamiento se basa en quelar el plomo casi específicamente con edetato disódico monocálcico, sin olvidar la penicilamida ni los nuevos ácido dimercapto-succínico y ácido dimercapto-propano-sulfónico, que han mejorado las opciones terapéuticas, son menos tóxicos y más fáciles de administrar. Se resalta la importancia del problema plomo como prioritario en salud pública y ocupacional, además de coadyuvar al grave problema ecológico actual.

    Endoscopic subureteral injection of dextranomer/hyaluronic acid has become an established alternative to long-term antibiotic prophylaxis or surgical treatment for vesicoureteral reflux. We evaluated the effectiveness of endoscopic injection of dextranomer/hyaluronic acid in intermediate and high grade vesicoureteral reflux in patients with complete duplex collecting systems. A total of 123 children underwent endoscopic correction of intermediate or high grade vesicoureteral reflux using injection of dextranomer/hyaluronic acid into complete duplex systems between 2001 and 2010. Vesicoureteral reflux was diagnosed by voiding cystourethrogram, and dimercapto-succinic acid scan was performed to evaluate the presence of renal scarring. Followup ultrasound and voiding cystourethrogram were performed 3 months after the outpatient procedure and renal ultrasound thereafter every 2 years. Mean followup was 6.7 years. Complete duplex systems were unilateral in 110 patients and bilateral in 13. Reflux severity in the 136 refluxing units was grade II in 1 (0.7%), III in 52 (38.2%), IV in 61 (44.9%) and V in 22 (16.2%). Dimercapto-succinic acid scan revealed renal functional abnormalities in 63 children (51.2%). Vesicoureteral reflux resolved after the first endoscopic injection of dextranomer/hyaluronic acid in 93 ureters (68.4%), after a second injection in 35 (25.7%) and after a third injection in 8 (5.9%). Febrile urinary tract infection developed in 5 patients (4.1%) during followup. No patient required ureteral reimplantation or experienced significant complications. Our results confirm the safety and efficacy of endoscopic injection of dextranomer/hyaluronic acid in eradicating intermediate and high grade vesicoureteral reflux in patients with complete duplex systems. We recommend this minimally invasive, 15-minute outpatient procedure as a viable option for treating intermediate and high grade vesicoureteral reflux in patients with complete duplex collecting systems

    Full Text Available We describe a case of a 58-year-old male with longstanding hypertension and Type 2 diabetes mellitus who developed sudden onset renal impairment. The first clue to the possible presence of amyloidosis in this case was provided by the radionuclide renal cortical scan performed with trivalent dimercapto succinic acid (Tc99m-DMSA-3, which revealed intense tracer uptake in the spleen suggesting amyloid deposit. Further workup to ascertain the cause of amyloidosis led to the diagnosis of multiple myeloma. We conclude that in cases of extra-renal or splenic accumulation of Tc99m-DMSA-3, a diagnosis of amyloidosis should be considered, in an appropriate clinical setting.

    Full Text Available Los antimoniales pentavalentes son la primera elección para el tratamiento de la leishmaniasis en Colombia pero la dosis ha tenido que incrementarse en mas de 600% para mantener su eficacia que en distintos reportes varía entre 93 y 25%, lo cual puede reflejar diferencias en la sensibilidad de los parásitos pero más probablemente incumplimiento de los esquemas recomendados. Los resultados de su empleo, así como la situación actual de pentamidina, anfotericina B y miltefosina son revisados y se hace una proyección sobre el desarrollo de la terapia en los próximos años. Para definir el real estado de la respuesta a los medicamentos anti-leishmaniásicos y prolongarles su vida útil, es indispensable establecer mecanismos de vigilancia de los factores implicados en el manejo de la enfermedad por lo que se plantea una estrategia de sitios centinela que permitan observaciones puntuales en el tiempo, en áreas específicas y sobre situaciones particulares.

    By the method of co-precipitation at constant pH was possible to perform the chemical synthesis of hybrid materials with HDLs bridging agent having as 2,5-dimercapto 1,3,4-thiadiazole (DMcT) and organic material capable of intercalating and polymerization (PAni). From the results of X-ray can be concluded that there was obtained the desired lamellar structure for collating the conductive polymer. From SEM micrographs, it was observed that the composite HDL / PDMcT / polyaniline, has a large number of compact crystallites various characteristic shapes and PDMcT polyaniline nanofibers. The VC of the ternary composite showed a significant increase of anodic charge. Given the results of adsorption of 98% of the pesticide in the composite polyaniline / PDMcT / HDL, it can contribute significantly to the removal of pesticide endosulfan contaminated water.(author)

    Full Text Available Descreve-se um caso de calazar grave resistente a dez cursos de antimonial petitavalente (glucantime à base de 20mg de Sb5/kg/dia, que respondeu favoravelmente ao sulfato de aminosidine intramuscular na dose de 20mg/kg/dia por 20 dias, repetido após 20 dias. O parasitismo esplénico passou de 50 parasitos por campo a 3 parasitos em 10 campos logo após a primeira série de sulfato de aminosidine, tornando-se negativo depois de sete meses. A melhora clínica foi imediata, com redução gradual da hepatoesplenomegalia, e desaparecimento 26 meses após. Neste período aumentou 13 kg. Após o uso de aminosidine a reação de Monténégro tornou-se positiva e as células mononucleares responderam quando estimuladas com antígenos de leishmânia.A grave kala-azar infection in a 14 years old boy is described. The leishmanial infection failed to respond to ten interrupted courses of glucantime of variable duration (14-56 days at a dose of 20mg Sb5/kg/day. However a favorable response ocurred to intramuscular aminosidine sulphate (20mg/kg/day for 20 days. This same regimen was repeated 20 days later. After the first treatment splenic puncture parasite density fell from 50 amastigotes per oil immersion field to 3 amastigotes in 10 fields. A further splenic puncture 7 months after treatment was negative. The marked hepatoesplenomegaly gradually resolved over 26 months follow up and he gained 13 kilogramas in weight. After aminosidine sulphate therapy his Montenegro reaction become positive and his lymphocytes responded to leishmania antigens.

All of them infected sandflies fed on the borders of the skin lesions before the treatment, suggesting that infected persons might act as reservoirs of infection for an indoor-bitting sandfly species. The negative results obtained by xenodiagnosis carried out after specific treatment of the same individuals indicated cure of the patients, and a reduction of risk for further intradomiciliary transmission.

    Full Text Available The work-up of a febrile urinary tract infection is generally performed to detect vesicoureteral reflux (VUR and its possible complications. The imaging modalities most commonly used for this purpose are renal-bladder ultrasound, voiding cystourethrogram and dimercapto-succinic acid scan. These studies each contribute valuable information, but carry individual benefits and limitations that may impact their efficacy. Biochemical markers are not commonly used in pediatric urology to diagnose or differentiate high-risk disease, but this is the emerging frontier, which will hopefully change our approach to VUR in the future. As it becomes more apparent that there is tremendous clinical variation within grades of VUR, the need to distinguish clinically significant from insignificant disease grows. The unfortunate truth about VUR is that recommendations for treatment may be inconsistent. Nuances in clinical decision-making will always exist, but opinions for medical versus surgical intervention should be more standardized, based on risk of injury to the kidney.

    Full text: Sensitive detection of acute peri-osseous scar tissue should be valuable for detection of partial ligamentous, tears and other common rheumatological conditions including back pain and ligamentous scars. Our aim was to investigate acute scar uptake of {sup 99m}Tc(V)-DMSA (dimercapto-succinic-acid), {sup 99m}Tc-DMAD (di- methyl-aminodiphosphonate) compared to {sup 99m}Tc-MDP (methylen-diphosphonate), the standard bone-scanning radiopharmaceutical. New Zealand white rabbits were anaesthetised and had 5-7cm of their mid-line abdominal wall surgically incised. At 24, 48, 72, 96 and 240 hours post surgery, 74 MBq (2 mCi) of the above radiopharmaceuticals were injected intravenously and scintigraphy performed 2.5 hours later. Relative count rate in scar is tabulated. In conclusion, the increased activity in the acute surgical site and lesser bone uptake confirmed that Tc (V)-DMSA and Tc-DMAD are superior to Tc- MDP for detection of new scar tissue in the region of bone. 1 tab.

Hemos evaluado la inmunogenia y reactogenia observadas al aplicarse las dosis primaria y de refuerzo de esta nueva vacuna a niños sanos y las hemos comparado con las observadas al aplicar un régimen de referencia a base de las vacunas autorizadas DTPw-HB (Tritanrix y antiHib (Hiberix en forma de inyecciones simultáneas. MÉTODOS: Llevamos a cabo un estudio aleatorizado y con doble enmascaramiento de septiembre de 1998 a agosto de 1999 para establecer la inmunogenia y reactogenia observadas al administrarles a niños sanos la nueva vacuna combinada pentavalente (DTPw-HB/Hib en una sola inyección, y compararlas con las observadas con el régimen de referencia. RESULTADOS: Se obtuvieron excelentes respuestas inmunitarias con ambos regímenes. Todos los niños vacunados en ambos grupos alcanzaron concentraciones séricas protectoras de anticuerpos antiFPR > 0,15 µg un mes después de recibir la dosis primaria. La vacuna combinada DTPw-HB/Hib no dio resultados inferiores a los obtenidos con las vacunas autorizadas en términos de los porcentajes de seroprotección, seropositividad y respuesta frente a todos los componentes antigénicos de la vacuna.     Mercaptothiadiazole ligands functionalized with thiol (2,5-dimercapto-1,3,4-thiadiazole (DMT)) and methyl (5-methyl-2-mercapto-1,3,4-thiadiazole (MMT)) groups capped onto citrate capped gold nanoparticles (C-AuNPs) by ligand exchange reaction was investigated by UV-vis spectroscopy, FT-IR spectroscopy and transmission electron microscopy (TEM) techniques. The surface plasmon resonance band at 522 nm for C-AuNPs was shifted to 530 nm after capping with DMT whereas an additional band was observed at 630 nm due to aggregation in addition to a shift in the band at 522 nm after capping of MMT onto C-AuNPs. Thus, capping of DMT onto C-AuNPs leads to the formation of stable AuNPs while capping of MMT leads to the formation of unstable AuNPs. FT-IR studies show that the citrate ions were completely replaced by both DMT and MMT ligands from the AuNPs. TEM images indicate that the size and shape of the AuNPs remain same after capping of these ligands.

    The case of a female 68 years old patient is described. Here, a chronic lead intoxication was diagnosed after a two year old medical history with increasing attacks of colic-like abdominal pain often described as life-threatening. After repeated hospitalizations and intensive search for the cause of the symptoms, porphyria and anemia was found to be a sign of a chronic lead poisoning. The blood lead concentrations were always about a level of 600 {mu}g/L. The source of exposure could not be found by now. Neither home inspection nor environmental investigations have shown a recent source of lead intake by the patient. However, a possible occupational source of lead exposure at a blast furnace was established by anamnesis for 1952 to 1962. Thus, osteoporosis induced lead mobilisation was suspected. Noticeable are the results of the six abdominal survey radiographies taken during hospitalization within one year; three radiographies were taken following clinical admission and three before discharge of the patient. In comparison, the course shows a chronic relapsing alimentary supply from metallic particles of unknown genesis. The patient was treated with the sodium salt of 2,3-dimercapto-1-propansulfonic acid (DMPS, Dimaval{sup TM}). She was free of complain afterwards. Following therapy, the blood lead concentrations fell under a level of 400 {mu}m/L, but after several weeks the lead level raised up to the original level of 600 {mu}g/L. (orig.) Muestra las alteraciones sufridas en el Calendario de Vacunación del 2012, para llegar a la reformulación de un Nuevo Calendario de Vacunación a partir del 2012. Señala las estrategias nacionales de vacunación enmarcadas en acciones de rutina, campañas y control de brotes. Hace referencia a los criterios de inclusión de nuevas vacunas referidos a aspectos: epidemiológicos, inmunológicos, tecnológicos, logística, socio-económicos, aprobación por los Comités y presupuesto. Registra las novedades ocurridas en el Sistema de Informaciones del Programa Nacional de Inmunizaciones, relacionadas con la situación actual, propuesta y Resolución Ministerial.     The effects of changes in sulfur-containing intracellular ligands on biliary excretion of cadmium were studied in rats. Injection of zinc or copper salts 24 h before intravenous injection of /sup 109/CdCl/sub 2/ (1 mg/kg Cd) decreased biliary excretion of Cd. Pretreatment with cysteine (25 mg/kg) had a similar effect. Depletion of intracellular thiol by injection of diethylmaleate had little effect. The effect of chelating agents on the pharmacokinetics of Cd depended on time of administration of the agents after exposure to Cd. When chelating agents were administered 1/2 h after Cd injection (before the synthesis of metallothionein), the thiol-containing agents (2,3-dimercapto-1-propanol (BAL), DL-penicillamine, N-acetylpenicillamine, and dithioerythritol increased the biliary excretion of Cd, while the carboxyl-containing ones (EDTA and nitrilotriacetate) increased the urinary excretion of Cd. BAL was the most effective chelating agent, but there was also an increase in the renal concentration of Cd. However, when these chelating agents were administered 24 h after Cd injection (after the synthesis of metallothionein), only BAL increased the biliary excretion of Cd. Renal and hepatic Cd concentrations decreased concurrently after BAL treatment.

  6. Risk factors for renal scarring in children with primary vesicoureteral reflux disease

    Full Text Available To determine the incidence of renal scarring among patients with primary vesicoureteral reflux (VUR and the possible risk factor(s, we studied 90 children (60 girls and 30 boys with VUR followed in the Pediatric Nephrology Unit at the Ege University Hospital from 1998 to 2003. All the patients were assessed for VUR grade by voiding cystoureterography and for presence of renal scarring by (99 m technetium dimercapto-succinic acid scintigraphy. All infants with VUR were given low-dose prophylactic antibiotics and followed-up until resolution of the reflux. Grade of reflux and number of urinary tract infection (UTI episodes (≥3 were found to be statistically significant risk factors for renal scarring (P 0.05. Similarly, there was no statistically significant difference of frequency of renal scarring among the different age groups (P >0.05. We conclude that recurrences of UTI and VUR severity are significant risk factors for renal scarring in children with VUR. Therefore, identification of VUR at an early age may offer the opportunity to prevent episodes of UTI and possible formation of renal scars that may result in end-stage renal failure.

    Freshly in situ prepared ferrite particles were used for the micro-solid phase extraction of arsenic species. When the separation was carried out at pH 8, inorganic arsenic (As(III) + As(V)) and monomethylarsonic acid (MMA) were retained in the magnetic material. A second aliquot was treated with 2,3 dimercapto propanol, leading to the retention of As(V)+MMA, while a third aliquot was first treated with sodium thiosulphate, in which case only inorganic arsenic passed to the solid phase. In all cases, the solid residue collected by a magnet was suspended in a dilute nitric acid solution containing Triton X-100 and introduced into the electrothermal atomizer to obtain the analytical signal of arsenic. The use of palladium as a chemical modifier allowed calibration to be carried out with aqueous standards. The detection limit was 0.02µgL -1 arsenic for a 10mL sample volume. The procedure was applied to waters and herbal infusions, and its reliability was evaluated by analyzing eleven certified reference materials for which speciation data are provided.     The stability of three 99m Tc radiopharmaceuticals in dependence on the experimental conditions and on the presence of some chemical stabiliser was tested. Examined were dimercapto succinate (DMS), pyrophosphate (PyP) and 2,3-dicarboxypropane-1,1-diphosphonate (DPD). The reliability and applicability of the preparation of the three radiopharmaceuticals from the inactive (technetium-cold) kit solutions were tested. Each kit was dissolved in saline (0.9% NaCl). The storage conditions of the inactive kit solutions before labelling were determined. The main problem is the stability of the reductant stannous ions which is very difficult to predict. Ascorbic or gentisic acid was added in order to stabilise the labelled radiopharmaceuticals and ensure their good quality. The best results were obtained by keeping the samples at -20 deg C. Although either stabiliser can be used, much lower concentrations of ascorbic acid are sufficient for an effective protection. Its concentrations of 12-60 μg/ml of the kit stabilise DMS and PyP for about 7-8 days. The solution of DPD was stable even without any stabiliser, presumably dut to the chemical nature of this complex. In routine practice, however, the procedure requires caution, and the staff should be very experienced in radiopharmacy procedures

  10. Usefulness of the dimercapto succinic pentavalent acid (99m Tc- Dmsa-V) in the diagnostic of the bone metastases illness

    The objective of this study was to determine the utility of 99m Tc-(V)- Dmsa whole body planar scan in the diagnostic of skeletal metastases. Nineteen patients were studied, having a recent 99m Tc-HDP bone scan reporting different pathologies (3 normal scans, 5 equivocal scans and 11 with disseminated bone metastases). 72 hours later, a whole body planar scan was obtained at 3 and 24 hours after the i.v. administration of 99m Tc-(V)-Dmsa. Counts per pixel were determined in regions of interest drawn over metastases sites and in normal tissue, and were correlated with the bone scan. Statistical analysis was carried out by using the Kruskal-Wallis analysis of variance followed by Mann Whitney U test. Other comparisons were done with Momios, chi square and t Student tests. 273 lesions were studied in the whole body bone scan and 184 lesions in the 99m Tc-(V)-Dmsa scintigraphy (t Student test n.s.). The tumor to normal tissue ratios were 3.3 (+/- 0.521) and the soft to normal tissue ratios were 1.01 (+/- 0.01), Mann Whitney p 99m Tc-(V)-Dmsa scintigraphy is an useful choice in the diagnostic of bone metastases when the whole body planar bone scan ( 99m Tc- HDP) is equivocal or abnormal.     This work presents a portable electrochemical system for the continuous monitoring of corrosion inhibitors in a wide range of matrices including ethanol, seawater and mineral oil following simple dilution of the samples. Proof-of-concept is demonstrated for the sensing of 2,5-dimercapto-1,3,5-thiadiazole (DMCT), an important corrosion inhibitor. Disposable screen-printed graphitic electrodes (SPGEs) associated with a portable batch-injection cell are proposed for the amperometric determination of DMCT following sample dilution with electrolyte (95% v/v ethanol + 5% v/v 0.1molL -1 H 2 SO 4 solution). This electrolyte was compatible with all samples and the organic-resistant SPGE could be used continuously for more than 200 injections (100µL injected at 193µLs -1 ) free from effects of adsorption of DMCT, which have a great affinity for metallic surfaces, and dissolution of the other reported SPGE inks which has hampered prior research efforts. Fast (180h -1 ) and precise responses (RSD < 3% n = 10) with a detection limit of 0.3µmolL -1 was obtained. The accuracy of the proposed method was attested through recovery tests (93-106%) and the reasonable agreement of results of DMCT concentrations in samples analyzed by both proposed and spectrophotometric (comparative) methods.   12. Visual test and colorimetric determination of gold(III with the use of indicator paper

    Full Text Available A visual test method has been proposed for the evaluation of the gold content. It is based on the linear dependence between the length of the colored zone on an indicator paper and gold(III concentration. Indicator paper was covered by a polymer film and was in contact with the solution tested along one edge during analysis. It was impregnated by 3-phenyl-2,6-dimercapto-1,4-thiopyrone or by precipitate of its complex with copper(II. Besides Cu(II can be replaced by the Au(III ion tested in the composition of this complex. The concentration ranges for Au(III determination were equal to 0.02−2 mg·L−1 or 4−590 mg·L−1, respectively, in the presence of excess of the transition and noble metals. It was demonstrated the effectiveness of the dynamic preconcentration of Au(III for the subsequent colorimetric determination on paper filter for the concentration range 0.005–0.3 mg·L−1 (DL 0.02 mg·L−1 by using a sample volume of 10 mL. The developed procedures were successfully utilized for the determination of gold in synthetic mixtures, in auriferous quartz (RSD < 5%, and in ore from Zyryanovsk’s mine (Kazakhstan with RSD < 8%.

    The adsorption capacity of a-chitosan and its modified form with succinic anhydride was compared with the traditional adsorbent active carbon by using the dye methylene blue, employed in the textile industry. The isotherms for both biopolymers were classified as SSA systems in the Giles model, more specifically iclass and subgroup 3. The dye concentration in the supernatant in the adsorption assay was determined through electronic spectroscopy. By calorimetric titration thermodynamic data of the interaction between methylene blue and the chemically modified chitosan at the solid/liquid interface were obtained. The enthalpy of the dye/chitosan interaction gave 2.47 {+-} 0.02 kJ mol-1 with an equilibrium constant of 7350 {+-} 10 and for the carbon/dye interaction this constant gave 5951 {+-} 8. The spontaneity of these adsorptions are reflected by the free Gibbs energies of -22.1 {+-} 0.4 and -21.5 {+-} 0.2 kJ mol-1, respectively, found for these systems. This new adsorbent derived from a natural polysaccharide is as efficient as activated carbon. However 97% of the bonded dye can be eluted by sodium chloride solution, while this same operation elutes only 42% from carbon. Chitosan is efficient in dye removal with the additional advantage of being cheap, non-toxic, biocompatible and biodegradable. (author)

    Full Text Available La pérdida de la eficacia, las dificultades para su administración y el incremento en la frecuenciay la severidad de los efectos secundarios de las sales de antimonio pentavalente han forzadola búsqueda de un medicamento que pueda remplazarlas en el tratamiento de la leishmaniasis.Los estudios adelantados con medicamentos inyectables, orales y tópicos han arrojadoresultados inconsistentes y de poca utilidad en el Nuevo Mundo. Desde 1998 lahexadecilfosfocolina (miltefosina viene siendo utilizada con éxito en India en enfermos conleishmaniasis visceral y en 1999 se iniciaron los estudios clínicos en Colombia en pacientescon leishmaniasis cutánea. A la fecha se han tratado mas de 2.500 pacientes en India(leishmaniasis visceral por L. donovani y Colombia (leishmaniasis cutánea por L. panamensisy se han logrado tasas de curación superiores a 91% a la dosis recomendada de 2,5 mg / kgpeso / día durante 28 días continuos, sin que haya diferencia en la respuesta entre pacientesnuevos y los que presentan fallas o recaídas después de recibir antimoniales. En Guatemalala respuesta fue de 53% ( 33% para L. braziliensis, 60% para L. mexicana mientras que la deL. tropica en Afganistán fue de 63%. Pacientes con leishmaniasis cutánea difusa, leishmaniasismucosa y con co-infección por HIV+, han tenido una buena respuesta inicial pero deberecordarse la tendencia a recaer que tienen estos pacientes. Entre el 35% al 60% de lospacientes presentan reacciones adversas gastrointestinales leves y 10% al 20% hacenelevación leve de transaminasas y creatinina.

None of these imaging strategies is universally accepted. The “top-down approach” (TDA aims at restricting the number of voiding cystourethrograms (VCUGs and its associated morbidity while identifying patients with clinically-significant reflux. In this approach, children presenting with febrile UTIs are acutely investigated with dimercapto-succinic acid (DMSA renal scans to identify patients with renal parenchymal inflammation. Those with evidence of renal affection are offered VCUG and late DMSA scan to identify VUR and permanent renal scarring, respectively. Although TDA could identify clinically-significant VUR with high sensitivity, it is not without limitations. The approach segregates patients based on the presence of DMSA cortical lesions omitting the morbidity and the economic burden of UTI. Additionally, some of DMSA lesions are attributed to congenital dysplasia and unrelated to UTI. Ionizing radiation exposure, financial costs, limited availability of DMSA scans in the acute setting, variability in interpreting the results and low yield of actionable findings on DMSA scans are some other limitations. In this review, we tried to address the drawbacks of the TDA and reinforce the value of patient-centered approach for VUR.

  16. Treating exposure to chemical warfare agents: implications for health care providers and community emergency planning.

    Current treatment protocols for exposure to nerve and vesicant agents found in the U.S. stockpile of unitary chemical weapons are summarized, and the toxicities of available antidotes are evaluated. The status of the most promising of the new nerve agent antidotes is reviewed. In the U.S. atropine and pralidoxime compose the only approved antidote regimen for organophosphate nerve agent poisoning. Diazepam may also be used if necessary to control convulsions. To avoid death, administration must occur within minutes of substantial exposure together with immediate decontamination. Continuous observation and repeated administration of antidotes are necessary as symptoms warrant. Available antidotes do not necessarily prevent respiratory failure or incapacitation. The toxicity of the antidotes themselves and the individualized nature of medical care preclude recommending that autoinjectors be distributed to the general public. In addition, precautionary administration of protective drugs to the general population would not be feasible or desirable. No antidote exists for poisoning by the vesicant sulfur mustard (H, HD, HT); effective intervention can only be accomplished by rapid decontamination followed by palliative treatment of symptoms. British anti-Lewisite (BAL) (2,3-dimercapto-1-propanolol) is the antidote of choice for treatment of exposure to Lewisite, another potent vesicant. Experimental water-soluble BAL analogues have been developed that are less toxic than BAL. Treatment protocols for each antidote are summarized in tabular form for use by health care providers.     This article reviews the clinical use of the metal chelators sodium 2,3-dimercapto-1-propanesulfonate (DMPS), meso-2,3-dimercaptosuccinic acid (DMSA), and calcium disodium edetate (CaEDTA, calcium EDTA) in overexposure and poisonings with salts of lead (Pb), mercury (Hg), and arsenic (As). DMSA has considerably lower toxicity than the classic heavy metal antagonist BAL (2,3-dimercaptopropanol) and is also less toxic than DMPS. Because of its adverse effects, CaEDTA should be replaced by DMSA as the antidote of choice in treating moderate Pb poisoning. Combination therapy with BAL and CaEDTA was previously recommended in cases of severe acute Pb poisoning with encephalopathy. We suggest that BAL in such cases acted as a shuttling Pb transporter from the intra- to the extracellular space. The present paper discusses if a combination of the extracellularly distributed DMSA with the ionophore, Monensin may provide a less toxic combination for Pb mobilization by increasing both the efflux of intracellularly deposited Pb and the urinary Pb excretion. Anyhow, oral therapy with DMSA should be continued with several intermittent courses. DMPS and DMSA are also promising antidotes in Hg poisoning, whereas DMPS seems to be a more efficient agent against As poisoning. However, new insight indicates that a combination of low-dosed BAL plus DMPS could be a preferred antidotal therapy to obtain mobilization of the intracerebral deposits into the circulation for subsequent rapid urinary excretion.

El tratamiento de elección de la leishmaniasis cutánea es el antimonio pentavalente con una eficacia cercana al 85%; sin embargo, debido a los serios efectos adversos y a la aparición cada vez más frecuente de resistencia al tratamiento, se hace necesaria la búsqueda de alternativas terapéuticas. Objetivo. Evaluar la eficacia terapéutica y la tolerancia del isotianato de pentamidina con un ensayo clínico piloto con pacientes con leishmaniasis cutánea producida por L. (V panamensis en Colombia. Materiales y métodos. Se incluyeron en el estudio 63 pacientes los cuales fueron tratados por vía intramuscular con 4 mg de isotianato de pentamidina/kg de peso/ interdiario por cuatro dosis, con seguimiento clínico por seis meses. Resultados. De los 63 paciente incluidos, 43 fueron evaluados durante los seis meses del estudio y de estos 37 (86% presentaron curación de todas sus lesiones mes y medio después de terminar el tratamiento. Cinco pacientes (11,6% fallaron al tratamiento por no cicatrización de las lesiones y uno presentó recaída. En general el tratamiento fue bien tolerado; los efectos adversos principales fueron dolor y edema en el sitio de aplicación, mareo y fiebre, de intensidad leve a moderada. No se presentaron hipoglucemia, hipotensión ni diabetes. Discusión. Los resultados del presente estudio validan los hallazgos de eficacia de la pentamidina para el tratamiento de la leishmaniasis cutánea en Colombia y en Brasil. Se destaca una baja frecuencia e intensidad de los efectos colaterales en la población civil donde no se encontró ningún paciente con efectos secundarios serios.

    Full Text Available Todo programa de inmunización tiene como meta alcanzar el más alto grado de protección contra las enfermedades inmunoprevenibles, con las más bajas tasas de reacciones adversas; constituyendo la vacunación un instrumento fundamental para la salud pública. El objetivo de este trabajo fue caracterizar los eventos adversos supuestamente atribuidos a la vacunación en la Isla de la Juventud, Cuba, entre enero/2013 y octubre/2016. Se realizó una investigación observacional descriptiva de corte transversal, puesta en práctica mediante la vigilancia pasiva. Se utilizó como fuente primaria para la recolección de datos la encuesta epidemiológica en los 507 pacientes que presentaron eventos adversos. Se comprobó que los años 2014 y 2015 superaron la tasa global para ese período: 34,6 por 10000 dosis administrada (DA, mientras que el área de salud “Leonilda Tamayo Matos” fue quien aportó la mayor tasa de eventos: 197 (39,7 por 10000 DA. Las vacunas con mayores notificaciones de reactogenicidad fueron: Pentavalente 280 (252,0 por 10000 DA, DPT 66 (168,4 por 10000 DA y la Antimeningocócica-BC: 86 (92,6 por 10000 DA. Se evidenció la mayor tasa de eventos adversos en el año 2014, predominando el área de salud “Leonilda Tamayo Matos”. Los eventos adversos más comunes fueron la fiebre y las reacciones locales. Los resultados obtenidos demuestran las bajas tasas de eventos adversos graves. Se demostró una vez más la confiabilidad del sistema y seguridad de las vacunas en Cuba.

  1. Longterm outcome of Macroplatique injection for treatment of vesicoureteral reflux in children

    Full Text Available Background: This study examined our experience with one year follow-up of 20 cases of vesicoureteric reflux in children after treatment with Macroplastique ® injection. Patient and Methods: A total of 20 children (31 ureters with primary grades II to V vesicoureteral reflux were treated with subureteral Macroplastique ® injection from 2010 to 2011 and followed for an average of 12 months (range 3 to 24. Vesicoureteral reflux was grade II in 3, III in 7, IV in 9 and V in 12 ureters. Each child underwent pre-operative voiding cystourethrography, abdominopelvic ultrasound, urine analysis/culture, S. creatinine and CBC. Dimercapto-succinic acid scan (DMSA scan and magnetic resonance urography (MRU were done in some patients. Voiding cystourethrography at 3 months was done to rule out persistent reflux. Results: Overall, reflux was corrected in 11 (35.5% ureters and 9 (45% children after a single injection. With repeat injection, reflux was corrected in 16 (51.6% ureters and 11 (55% children, reflux improved/downgrade in 4 (12.9% ureters and 2 (10% children. Correction by grade was 100%, 100%, 9.7%, 9.7% for grades II to V, respectively. There were no surgical complications. None of the cured patients had recurrent reflux during follow-up. There were 9 (45% children who required open ureteral re-implantation for failed injection. Conclusion: Sub-ureteral Macroplastique ® injection therapy could be a primary treatment for low grade VUR (grade III or less in children because it is simple, safe, effective, less invasive, decreased.

    We retrospectively assessed the incidence of and risk factors for febrile urinary tract infection in children during active surveillance after early discontinuation of antibiotic prophylaxis. We retrospectively evaluated 9 females and 61 uncircumcised males diagnosed with primary vesicoureteral reflux before age 1 year who had persistent reflux on followup voiding cystourethrogram and were subsequently followed under active surveillance without continuous antibiotic prophylaxis. Patients with secondary vesicoureteral reflux or associated urological abnormality were excluded. Clinical outcomes, including incidence of febrile urinary tract infection and new scar formation, were evaluated. Risk factors for febrile urinary tract infection were also analyzed. Mean age at stopping continuous antibiotic prophylaxis was 21 months, and mean followup was 61 months. During active surveillance 21 patients had febrile urinary tract infection, and the 5-year infection-free rate under active surveillance was 67.5%. One or 2 foci of minimal new scarring developed in 4 of 16 patients who underwent followup dimercapto-succinic acid scan after febrile urinary tract infection. On multivariate analysis dilated vesicoureteral reflux on followup voiding cystourethrogram was the only significant risk factor for febrile urinary tract infection. This study revealed that about two-thirds of patients with persistent vesicoureteral reflux were free of febrile urinary tract infection during 5 years of active surveillance. Those with dilated vesicoureteral reflux on followup voiding cystourethrogram are at significantly greater risk for febrile urinary tract infection. Accordingly active surveillance, especially in patients with nondilated vesicoureteral reflux on followup voiding cystourethrogram, seems to be a safe option even in children who have not yet been toilet trained.     Full Text Available Se realizó un estudio descriptivo y transversal de 608 lactantes de 2 áreas de salud integral comunitaria, registrados en el Hospital "Dr. José María Bengoa" de Sanare, estado venezolano de Lara, durante el 2010, para determinar el cumplimiento del esquema de vacunación en estos. El grupo etario mayormente representado fue el de 7-12 meses y el de 29 días de nacido, lo cual mostró una labor educativa favorable en el primer nivel de asistencia sanitaria. Se observaron dificultades en la comunicación y accesibilidad a la correspondiente área de salud, así como también con la cobertura de atención, tanto del personal profesional cubano como del venezolano; de igual manera, el esquema de vacunación con Bacillus Calmette-Guérin, pentavalente y contra la parotiditis, la rubéola y el sarampión no era el estimado, a pesar de los avances obtenidos, cuyos principales factores negativos fueron la carencia de vacunas, las oportunidades perdidas y la poca accesibilidad al vacunatorio.A descriptive and cross-sectional study was carried out in 608 infants from 2 community comprehensive health areas, recorded in "Dr. José María Bengoa" Hospital of Sanare , Venezuelan state of Lara, during 2010 to determine the compliance with the vaccination schedule in these infants. The most represented age group was that of 7-12 months and 29 days old, which showed a favorable educational work on the first level of health care. There were difficulties in communication and accessibility to the corresponding health area, as well as with the care coverage both by Cuban and Venezuelan professional personnel. Similarly, the vaccination schedule with Bacillus Calmette-Guérin, pentavalent and measles-mumps-rubella vaccine was not as estimated in spite of the advances achieved, which main negative factors were the lack of vaccines, missed opportunities and poor accessibility to the vaccination.

    Full Text Available The first choice of treatment for American cutaneous leishmaniasis is the pentavalent antimonial drug. Although it has been shown that this treatment is mostly effective and indicated, some disadvantages should be taken into account such as side effects, long term treatment inconveniences and counter-indication for patients suffering from cardiopathy, nephropathy; yet, aging, pregnancy and other conditions. With the advent of the vaccine anti-American cutaneous leishmaniasis as a prophylactic measure, studies on therapy using the vaccine associated or not with other drugs have been performed by many investigators and it is currently among the alternative treatments and prevention measures for American cutaneous leishmaniasis. In conclusion, the association between antimony and vaccine (immunochemotherapy showed the same cure rate when compared with the standard treatment (100% and it was also able to reduce the salt volume in 17.9% and treatment length from 87 to 62 days, decreasing side effects.O tratamento de primeira escolha para leishmaniose tegumentar americana é o antimonial pentavalente. Embora este tratamento seja na maioria das vezes efetivo e indicado, devem ser consideradas as desvantagens tais como efeitos colaterais, longa duração do tratamento e contra-indicação para cardiopatas, nefropatas, idosos, grávidas e outras condições. Com o advento da vacina antileishmaniose tegumentar americana para fins profiláticos e terapêuticos, associando-a ou não a outros fármacos, muitas pesquisas têm sido desenvolvidas, sendo a vacina a principal entre os atuais recursos no tratamento e prevenção da leishmaniose tegumentar americana. Em conclusão, a associação do antimônio com a vacina (imunoquimioterapia apresentou o mesmo índice de cura em relação ao tratamento padrão (100%, e ainda reduziu o volume do sal em 17,9% e o tempo de cura significativamente, de 87 para 62 dias; conseqüentemente, reduzindo os efeitos colaterais.

    Full Text Available São apresentados quatro casos de leishmaniose em pacientes com SIDA, sendo dois de leishmaniose visceral e dois com forma cutâneo-mucosa. Lesões cutâneas e de mucosa oral, disseminadas, estavam presentes nos pacientes com a forma tegumentar da doença. Febre prolongada, hepatoesplenomegalia e pancitopenia foram as manifestações principais da forma visceral. A contagem de linfócitos T CD4+ era baixa em todos os casos. A pesquisa de leishmanias foi positiva no aspirado de medula óssea e na biópsia de lesões cutâneas e mucosas. Os pacientes responderam ao tratamento com antimoniais pentavalentes e com a anfotericina B. Poucos casos de coinfecção leishmaniose e HIV têm sido descritos em nosso meio. Apesar da ampla expansão de ambas, até agora suas áreas de distribuição geográfica teve pouca superposição. Os autores recomendam a inclusão desta parasitose no diagnostico diferencial das doenças oportunistas que acometem indivíduos com SIDA.Few cases of concurrent leishmaniasis and HIV infection have been reported in Brazil, despite both infections being in expansion. Two cases of visceral leishmaniasis and two cases of mucocutaneous leishmaniasis are discussed. Disseminated skin and oral lesions were found in the patients with the cutaneous form of the disease. Prolonged fever, hepatosplenomegaly and pancytopenia were the main manifestations of the visceral form. The CD4 T lymphocyte count was low in all cases. Direct examination of bone marrow aspirate for leishmania and biopsy of cutaneous lesions are the techniques of choice to confirm diagnosis. Pentavalent antimonials and amphotericin B are preferred drugs for the treatment of leishmaniasis, including patients with AIDS. The authors recommend the inclusion of this parasitosis in the differential diagnosis of opportunistic diseases in patients with AIDS.

  9. Implications of the Differential Toxicological Effects of III-V Ionic and Particulate Materials for Hazard Assessment of Semiconductor Slurries.

    Because of tunable band gaps, high carrier mobility, and low-energy consumption rates, III-V materials are attractive for use in semiconductor wafers. However, these wafers require chemical mechanical planarization (CMP) for polishing, which leads to the generation of large quantities of hazardous waste including particulate and ionic III-V debris. Although the toxic effects of micron-sized III-V materials have been studied in vivo, no comprehensive assessment has been undertaken to elucidate the hazardous effects of submicron particulates and released III-V ionic components. Since III-V materials may contribute disproportionately to the hazard of CMP slurries, we obtained GaP, InP, GaAs, and InAs as micron- (0.2-3 μm) and nanoscale (particles for comparative studies of their cytotoxic potential in macrophage (THP-1) and lung epithelial (BEAS-2B) cell lines. We found that nanosized III-V arsenides, including GaAs and InAs, could induce significantly more cytotoxicity over a 24-72 h observation period. In contrast, GaP and InP particulates of all sizes as well as ionic GaCl3 and InCl3 were substantially less hazardous. The principal mechanism of III-V arsenide nanoparticle toxicity is dissolution and shedding of toxic As(III) and, to a lesser extent, As(V) ions. GaAs dissolves in the cell culture medium as well as in acidifying intracellular compartments, while InAs dissolves (more slowly) inside cells. Chelation of released As by 2,3-dimercapto-1-propanesulfonic acid interfered in GaAs toxicity. Collectively, these results demonstrate that III-V arsenides, GaAs and InAs nanoparticles, contribute in a major way to the toxicity of III-V materials that could appear in slurries. This finding is of importance for considering how to deal with the hazard potential of CMP slurries.

    In recent years the endoscopic injection of dextranomer/hyaluronic acid has become an established alternative to long-term antibiotic prophylaxis and the surgical management of vesicoureteral reflux. We determined the safety and effectiveness of the endoscopic injection of dextranomer/hyaluronic acid as first line treatment for high grade vesicoureteral reflux. Between 2001 and 2010, 1,551 children (496 male, 1,055 female, median age 1.6 years) underwent endoscopic correction of intermediate and high grade vesicoureteral reflux using dextranomer/hyaluronic acid soon after the diagnosis of vesicoureteral reflux on initial voiding cystourethrogram. Vesicoureteral reflux was unilateral in 761 children and bilateral in 790. Renal scarring was detected in 369 (26.7%) of the 1,384 patients who underwent dimercapto-succinic acid imaging. Reflux grade in the 2,341 ureters was II in 98 (4.2%), III in 1,340 (57.3%), IV in 818 (34.9%) and V in 85 (3.6%). Followup ultrasound and voiding cystourethrogram were performed 3 months after the outpatient procedure, and renal ultrasound was performed annually thereafter. Patients were followed for 3 months to 10 years (median 5.6 years). Vesicoureteral reflux resolved after the first, second and third endoscopic injection of dextranomer/hyaluronic acid in 2,039 (87.1%), 264 (11.3%) and 38 (1.6%) ureters, respectively. Febrile urinary tract infections developed during followup in 69 (4.6%) patients. None of the patients in the series needed reimplantation of ureters or experienced any significant complications. Our results confirm the safety and efficacy of the endoscopic injection of dextranomer/hyaluronic acid in the eradication of high grade vesicoureteral reflux.     Technetium-99 m is the most useful radionuclide in diagnostic imaging procedures in Nuclear Medicine, more than 80 percent of radiopharmaceuticals are {sup 99m}Tc-labeled compounds. {sup 99m}Tc-DMSA(V) has been used for imaging of soft tissue, head and neck tumors. It shows a particularly high specificity for medullary thyroid carcinoma and bone metastases in a variety of cancers. Biodistribution studies of {sup 188}Re-DMSA(V) have shown that its general pharmacokinetic properties are similar to that of {sup 99m}Tc-DMSA(V), so this agent could be used for targeted radiotherapy of these tumors. The aim of this work is the development of methods of labeling DMSA(V) with {sup 99m}Tc and {sup 188}Re. {sup 99m}Tc-DMSA(V) can be prepared by two methods. One of them is the indirect one, through the use of a commercial kit of DMSA (III), by adjusting the pH from 2.5 to {approx} 8.5 with NaHCO{sub 3}. This method was evaluated and optimized presenting high labeling yields. The other method is the direct one, through the preparation of a lyophilised kit ready for labeling with {sup 99m}Tc, being the method of interest of this work, due to the easy of its clinical use. The most adequate formulation of the kit was: 1.71 mg of DMSA, 0.53 mg of SnCl{sub 2}.2H{sub 2}O and 0.83 mg of ascorbic acid (pH 9). Labeling yields higher than 95% were achieved labeling this kit with 1 to 2 m L of {sup 99m}Tc with activities up to 4736 MBq (128 mCi). The kit was stable up to 6 months and biodistribution studies confirmed the quality of the DMSA (V) labeled with {sup 99m}Tc using this kit. The reduction potential of Re is lower than the one for Tc, so the labeling conditions of {sup 188}Re-DMSA(V) are different from the ones used for {sup 99m}Tc- DMSA(V). {sup 188}Re-DMSA(V) is prepared in acid solution, that makes it possible to use the DMSA (III) commercial kit developed for labeling with {sup 99m}Tc, prepared in pH 2.5, for labeling with {sup 188}Re. Labeling yields higher than 95% were achieved with this methodology, with a rection time of 30 minutes at 100 deg C using no more than 1 m L of {sup 188}ReO{sub 4}{sup -}. Another method of preparing {sup 188}Re-DMSA(V) was also evaluated, using a liquid kit containing 2.5 mg of DMSA, 1.00 mg of SnCl{sub 2}.2H{sub 2}O and 30 mg of sodium oxalate at pH 5.   12. Color discrimination impairment in workers exposed to mercury vapor.

    To study color discrimination impairment in workers exposed to elemental mercury (Hg) vapor. Twenty-four male workers from a chloralkali plant exposed to Hg vapor, aged 42+/-9.8 years, duration of exposure 14.7+/-9.7 years, were examined. The 8h TWA air-borne Hg concentration in workplace was 59 microg/m(3); mean Hg urinary excretion (HgU) was 20.5+/-19.3 microg/g creatinine; mean Hg urinary excretion after the administration of a chelating agent, sodium 2,3-dimercapto-1-propane-sulfonate (DMPS), was 751.9+/-648 microg/48h. Twenty-four age- and gender-matched control subjects were compared. Visual acuity, alcohol intake, smoking habits, and history of diseases or drugs potentially influencing color vision were registered. The Lanthony 15-Hue desaturated test (L-D15-d) was used to assess color vision. The results were expressed quantitatively as Bowman's Color Confusion Index (CCI), and qualitatively according to Verriest's classification of acquired dyschromatopsias. The CCI was significantly higher in the exposed group than in the control (mean CCI 1.15 versus 1.04; P=0.04). The proportion of subjects with errorless performance on the Lanthony test was significantly lower in the Hg exposed group compared to referents (52% versus 73%; P=0.035). The exposed group showed higher frequency of type III dyschromatopsias (blue-yellow confusion axis) in comparison with the control group (12.5% versus 8.3%), however, the difference did not reach statistical significance. Multiple regression did not show any significant relationship between the CCI, and age, alcohol consumption, or measures of exposure. In agreement with previous studies by Cavalleri et al. [Toxicol. Lett. 77 (1995) 351; Environ. Res. Sec. A 77 (1998) 173], the results of this study support the hypothesis that exposure to mercury vapor can induce sub-clinical color vision impairment.     Full Text Available &lt;b&gt;Purpose:&lt;/b&gt; Early diagnosis and treatment of acute pyelonephritis in children is of special importance in order to prevent serious complications. This study was conducted to determine the diagnostic value of serum interleukin (IL-6 and IL-8 in children with acute pyelonephritis. &lt;b&gt;Methods:&lt;/b&gt; Eighty seven patients between 1 month to 12 years old with urinary tract infection (UTI were divided into 2 groups based on the result of 99m-technetium dimercapto-succinic acid (DMSA renal scan: acute pyelonephritis (n=37 and lower UTI (n=50 groups. White blood cell (WBC count, neutrophil (Neutl count, erythrocyte sedimentation rate (ESR, C-reactive protein (CRP concentration, platelet count, and serum IL-6 and IL-8 concentrations of both groups were measured and compared . &lt;b&gt;Results:&lt;/b&gt; There was a significant difference between two groups regarding WBC count, Neutl count, ESR, and CRP concentration (P&lt;0.05. In addition, the difference between the two groups regarding serum IL-6 and IL-8 concentrations was not significant (IL-6, 60 and 35.4 pg/mL and IL-8, 404 and 617 pg/mL, respectively. The sensitivity and specificity of serum IL-6 and IL-8 for diagnosis of acute pyelonephritis were 73%, 42% and 78%, 32%, respectively. Sensitivity, specificity, negative and positive predictive values of serum IL-6 and IL-8 were less than those of acute phase serum reactants such as CRP. &lt;b&gt;Conclusion:&lt;/b&gt; This study showed that there was no significant difference between acute pyelonephritis and lower UTI groups regarding serum IL-6 and IL-8 levels. Therefore, despite confirming results of previous studies, it seems that IL-6 and IL-8 are not suitable markers for differentiating between acute pyelonephritis and lower UTI.

  14. Successful Splenectomy for Hypersplenism in Wilson's Disease: A Single Center Experience from China.

    Splenomegaly and pancytopenia are common in Wilson's disease (WD) and splenectomy is one of the conventional treatments for splenomegaly and the associated pancytopenia. However, splenectomy remained controversial for hypersplenism in WD as it was reported that splenectomy leaded to serious emotional and neurological deterioration in WD patients with hypersplenism. In the current study, we present our experiences in 70 WD patients with hypersplenism who had undergone splenectomy, outlining the safety and efficacy of splenectomy in WD. The clinical database of 70 WD patients with hypersplenism who had undergone splenectomy in our hospital between 2009 and 2013 were reviewed and followed-up regularly. Before splenectomy, all the patients accepted a short period of anti-copper treatment with intravenous sodium 2, 3-dimercapto-1-propane sulfonate (DMPS). All the patients demonstrated a marked improvement in platelet and leucocyte counts after splenectomy. No severe postoperative complication was observed. In particular, none of the 37 patients with mixed neurologic and hepatic presentations experienced neurological deterioration after splenectomy, and none of the patients with only hepatic presentations newly developed neurological symptoms. During the one year follow-up period, no patient presented hepatic failure or hepatic encephalopathy, no hepatic patient newly developed neurological presentations, and only 3 patients with mixed neurologic and hepatic presentations suffered neurological deterioration and these 3 patients had poor compliance of anti-copper treatment. Quantative analysis of the neurological symptoms in the 37 patients using the Unified Wilson's Disease Rating Scale (UWDRS) showed that the neurological symptoms were not changed in a short-term of one week after splenectomy but significantly improved in a long-term of one year after splenectomy.     Full Text Available Pentavalent antimony, as meglumine antimoniate (Glucantime® or sodium stibogluconate (Pentostam® , is the main treatment for leishmaniasis, a complex of diseases caused by the protozoan Leishmania, and an endemic and neglected threat in Brazil. Despite over half a century of clinical use, their mechanism of action, toxicity and pharmacokinetic data remain unknown. The analytical methods for determination of antimony in biological systems remain complex and have low sensitivity. Radiotracer studies have a potential in pharmaceutical development. The aim of this study was to obtain a radiotracer for antimony, with suitable physical and biological properties. Meglumine antimoniate was neutron irradiated inside the IEA-R1 nuclear reactor, producing two radioisotopes 122Sb and 124Sb, with high radionuclidic purity and good specific activity. This compound showed the same antileishmanial activity as the native compound.     Full Text Available Se realizó un estudio descriptivo, transversal y retrospectivo para caracterizar los eventos adversos temporalmente asociados con las vacunas que se emplean en la prevención y el control de las enfermedades infecciosas, y que fueron notificados a la Unidad Coordinadora Nacional de Farmacovigilancia entre los años 2006-2007. Se determinó su comportamiento de acuerdo con la edad, sexo, procedencia de la notificación, personal que reporta, localización y severidad. Se identificaron además los principales eventos reportados y las vacunas implicadas en su aparición. La fiebre constituyó el 60 % del total de eventos notificados. Estos últimos se distribuyeron de igual forma entre uno y otro sexos, en tanto los lactantes resultaron ser los más afectados (46,8 %. Se destacó en el reporte la Atención Primaria de Salud con 812 notificaciones. Los médicos fueron los profesionales que más reportaron (36 %. El comportamiento en cuanto a severidad no se diferenció de lo reportado en la literatura, pues afortunadamente predominaron los eventos leves (66,4 %. Sin embargo, contrario a lo que se esperaba, los eventos sistémicos fueron los de mayor cuantía (80,2 %.     Introduction: renal artery stenosis can lead to renovascular hypertension; however, the detection of stenosis alone does not guarantee the presence of renovascular hypertension. Renovascular hypertension depends on activation of the renin-angiotensin system, which can be detected by functional tests such as captopril renal scintigraphy. A method that allows direct measurement of the baseline and post-captopril glomerular filtration rate using chromium-51 labeled ethylenediamine tetraacetic acid ({sup 51}Cr-EDTA) could add valuable information to the investigation of hypertensive patients with renal artery stenosis. The purposes of this study were to create a protocol to measure the baseline and post-captopril glomerular filtration rate using {sup 51}Cr-EDTA, and to verify whether changes in the glomerular filtration rate permit differentiation between hypertensive patients with and without renal artery stenosis. Methods: this prospective study included 41 consecutive patients with poorly controlled severe hypertension. All patients had undergone a radiological investigation of renal artery stenosis within the month prior to their inclusion. The patients were divided into two groups: patients with (n=21) and without renal artery stenosis, (n=20). In vitro glomerular filtration rate analysis ({sup 51}Cr-EDTA) and {sup 99m}Tc-DMSA scintigraphy were performed before and after captopril administration in all patients.     Myocardial perfusion single-photon emission tomography (SPET) performed with cationic technetium-99m complexes indicates ischaemic areas as cold lesions. By contrast, nitroimidazole derivatives labelled with fluorine-18 or 99m Tc have recently shown promising results for hot spot imaging of ischaemic myocardium. This study evaluates 99m TcO(BAT-NI), a new 99m Tc complex comprising the nitroimidazole ligand, 2,10-dimercapto-2, 10-dimethyl-4, 8-diaza-6-[4-(2-nitroimidazolyl)butyl]undecane, in a low-flow in vivo model of myocardial ischaemia in thoracotomised rats. To elucidate the influence of the 2-nitroimidazole group on ischaemia-induced uptake, comparisons with ligand derivatives were performed where (a) the 2-nitro group was deleted [ 99m TcO(BAT-I)], (b) the 2-nitroimidazole functionality was replaced by a Br atom [ 99m TcO(BAT-Br)] and (c) the 99m TcO(BAT) moiety was replaced by an iodine-125 iodophenoxybutyl ligand ( 125 IP-NI). The radiolabelled compounds were i.v. injected 15 min after reducing resting myocardial blood flow by 50-60% and the uptake of radioactivity was assessed 90 min post injection. Autoradiography of left ventricular short-axis slices showed median uptake ratios of ischaemic/non-ischaemic myocardium (I/N) of 3.4, 4.5 and 3.4 for 99m TcO(BAT-NI), 99m TcO(BAT-I) and 99m TcO(BAT-Br), respectively. In contrast, 125 IP-NI was not preferentially taken up by ischaemic myocardium. Accumulation of 99m TcO(BAT-NI) in ischaemic heart regions was comparable to that in the liver. Biodistribution studies showed a median uptake of 0.65% ID/g of 99m TcO(BAT-NI) in ischaemic tissue and an I/N of 3.3. On planar images of the thorax and upper abdomen the ischaemic hearts were visualised faintly; the median heart to lung count ratio for 99m TcO(BAT-NI) was 1.7, and the median heart to liver count ratio was 1.0.     Full Text Available OBJETIVO: Analizar los principales indicadores asociados a la mortalidad materna y la mortalidad en niños menores de 1 año y evaluar su nivel de cobertura y de variabilidad entre las distintas entidades federativas de México. MÉTODOS: Se estudiaron ocho intervenciones dirigidas a la atención primaria de la salud materna e infantil (variables: esquema de vacunación completo, vacuna de sarampión y vacuna pentavalente en menores de 1 año; inicio temprano de la lactancia materna; atención prenatal con al menos una revisión por personal capacitado; prevalencia del uso de anticonceptivos en mujeres casadas en edad fértil; atención obstétrica del parto por personal capacitado y aplicación de toxoide tetánico (TT en embarazadas. Se calculó el promedio y desviación estándar de la cobertura nacional para cada variable. Al interior de cada entidad federativa se determinó la proporción de municipios con alta, media y baja marginación. Se ordenaron las entidades federativas de mayor a menor, con base en la proporción de sus municipios con alta marginación y se dividieron en quintiles. Se calculó la desigualdad absoluta mediante la diferencia observada y la desigualdad relativa mediante la razón para cada una de las variables estudiadas. RESULTADOS: El promedio de cobertura nacional de las ocho variables estudiadas varió entre 86,5% y 97,5%, encontrándose que la cobertura más baja fue la aplicación de TT en mujeres embarazadas y la más alta, la vacuna contra el sarampión en menores de 1 año. La atención obstétrica, la prevalencia del uso de anticonceptivos y la revisión prenatal fueron las variables con coberturas menos equitativas.     Full Text Available This paper is a review of the history, synthesis and application of organophosphorus compounds, especially of those of pentavalent phosphorus, such as phosphoramidates, phosphorothioates, phosphonates and phosphonic acids with insecticide and anticancer activities. The organophosphorus compounds with agrochemical applications show great structural variety, They include not only insecticides, but also fungicides, herbicides, and others. The large variety of commercially available organophosphorus pesticides is remarkable. Even more interesting is the high efficiency of some organophosphorus compounds as anticancer agents such as cyclophosphamide and its derivatives.

    Full Text Available In Colombia, pentavalent antimonials and miltefosine are the drugs of choice for the treatment of cutaneous leishmaniasis; however, their toxicity, treatment duration, (treatment adherence problems, cost, and decreased parasite sensitivity make the search for alternative treatments of American cutaneous leishmaniasis necessary. Based on the results found in a controlled, open, randomized, phase III clinical trial, the efficacy and safety of miltefosine was compared to that of thermotherapy for the treatment of cutaneous leishmaniasis in Colombia. Adult patients from the Colombian army participated in the study; they received either 50 mg of miltefosine three times per day for 28 days by the oral route (n = 145 or a thermotherapy (Thermomed® application of 50 °C for 30 seconds over the lesion and surrounding area (n = 149. Both groups were comparable with respect to their sociodemographic, clinical, and parasitological characteristics. The efficacy of miltefosine by protocol and by intention to treat was 70% (85/122 patients and 69% (85/145 patients, respectively. The adverse effects were primarily gastrointestinal for miltefosine and pain at the lesion site after treatment for thermotherapy. No statistically significant difference was found in the efficacy analysis (intention to treat and protocol between the two treatments.     We have studied the electrocatalytic effects of polythiophene-based conducting polymers toward the redox reactions of the dilithium salt of the thiadiazole-based dithiol compound 2,5-dimercapto-1,3,4-thiodiazole (DMcT-2Li) via cyclic voltammetry (CV), rotating-disk electrode voltammetry, and electrochemical impedance spectroscopy (EIS). We have found that the electrocatalytic activity of the conducting polymers is strongly influenced by the potential range over which the polymers are electrically conductive (i.e., window of conductivity), which was tuned by employing different electron-donating groups at the 3- or 3,4-positions of polythiophene (PTh). Both poly(3,4-ethylenedioxythiophene) (PEDOT) and poly(3,4-propylenedioxythiophene) (PProDOT), whose windows of conductivity exhibited a good overlap with the formal potential for the dimerization process of DMcT-2Li; E0′ d (?0.54 V versus Ag/Ag+) exhibited electrocatalytic activity toward both the oxidation and reduction processes of DMcT-2Li. On the other hand, PTh, poly(3-methylthiophene) (PMTh), and poly(3,4- dimethoxythiophene) (PDMTh), whose windows of conductivity did not overlap with E0′d, did not exhibit electrocatalytic activity. The standard charge transfer rate constants for the dimerization process of DMcT-2Li at PEDOT, PProDOT, and PDMTh film-modified glassy carbon electrodes (GCEs) were estimated to be 7.4 - 10?4, 3.2 - 10?4, and 6.9 - 10?5 cm/s while the rate constant was 6.3 - 10?5 cm/s at an unmodified GCE. Moreover, EIS studies for PEDOT, PProDOT, and PDMTh film-modified GCEs indicated the smallest charge transfer resistance for a PEDOT film and highest for a PDMTh film at E0′d, indicating that the higher the electrical conductivity of a film at E 0′d the higher the electrocatalytic activity toward the redox reactions of DMcT-2Li.     Full Text Available A Leishmaniose Tegumentar Americana é uma infecção causada por uma variedade de espécies de Leishmania, é transmitida ao homem por flebotomíneos. Os antimoniais pentavalentes são utilizados na quimioterapia dessa doença; no entanto, esses fármacos provocam uma série de efeitos colaterais, além de apresentar altos índices de toxicidade. Na busca por novos agentes leishmanicidas, avaliamos in vitro o extrato do látex de Croton lechleri (Sangue de dragão frente a formas promastigotas de Leishmania amazonensis e Leishmania guyanensis. O extrato foi obtido através da desidratação do látex de C. lechleri e diluído em solução 10% de etanol em salina tamponada com fosfato. As promastigotas de Leishmania foram cultivadas juntamente com o extrato nas concentrações de 6,25; 12,5 e 25μg/ mL por 72 horas. O extrato apresentou eficácia em todas as concentrações testadas apresentando um IC 50 (concentração inibitória de 5,04μg/mL para L. amazonensis e IC 50 de 9,05μg/mL para L. guyanensis. Foi realizada a avaliação citotóxica em células J774 nas mesmas concentrações usadas no ensaio leishmanicida, porém a concentração de 25μg/mL apresentou  índice de toxicidade de aproximadamente 50% para a célula hospedeira. Os testes realizados mostraram-sepromissores, pois o extrato testado também foi capaz de inibir o crescimento de promastigotas de L. amazonensis após ensaio de infecção com células J774.Palavras-chave: Croton lechleri.     Full Text Available Resumen: Antecedentes: La recomendación vigente de vacunación en prematuros es iniciar a la edad cronológica independientemente del peso y edad gestacional al nacer. Objetivo: Describir el estado de vacunación de los infantes prematuros menores de 1500 g pertenecientes al programa de seguimiento pediátrico del INPer. Material y métodos: Estudio de cohorte descriptico retrospectivo y transversal. Se incluyeron infantes que acudieron a consulta de junio del 2008 a junio del 2009 con fecha de nacimiento del 2004-2008, se obtuvo de la cartilla nacional de vacunación la fecha de la administración de la vacuna y se interrogó la causa del retraso de la administración del mismo de acuerdo al esquema vigente. Resultados: Se incluyeron 158 infantes. El 78.5% recibieron todas sus vacunas en el sector público. Transcurrieron 54.5 ± 30.2 días entre egreso y la primera aplicación de algún inmunógeno. La cobertura de vacunación contra influenza con dos dosis en el primer año de vida fue: 18.9%, contra rotavirus dos dosis: 59.5%, pentavalente acelular tres dosis: 73.5%, neumococo tres dosis: 28.2%, BCG: 98.1%, SRP: 100% y hepatitis B tres dosis: 77.8%. Todos los inmunógenos de administraron en forma atrasada. Las tres causas más frecuentes de rechazo en la vacunación fueron: peso bajo y/o prematurez, desabasto del biológico y antecedente de transfusión. Conclusiones: Los infantes prematuros mostraron retraso en la administración de todos los inmunógenos en relación con la recomendación del esquema nacional de vacunación, se debe de revisar las indicaciones vigentes para la administración de inmunógenos en esta población.     Full Text Available A fermentação do vinho é um processo microbiológico complexo que requere a presença de leveduras adaptadas a condições de stresse. No ambiente celular de organismos aeróbios ocorrem naturalmente espécies reactivas de oxigénio (ROS como subprodutos da respiração mitocondrial. A elevada reactividade destas espécies químicas pode gerar danos moleculares que, em alguns casos, levam à morte celular. Em condições fisiológicas normais ou como resposta ao stresse oxidativo, a célula pode desencadear respostas adaptativas que envolvem mecanismos antioxidantes como os enzimas glutationo redutase (GR; EC 1.6.4.2 e catalases T (CAT T; EC 1.11.1.6 e A (CAT A; EC 1.11.1.6. O vanádio, um metal pesado presente em alguns fitofármacos, pode também com portar-se como um gerador de ROS, alterando o estado redox intracelular e exercendo efeitos nocivos em leveduras expostas a quantidade excessiva deste elemento. O principal objectivo deste trabalho foi comparar o efeito do metavanadato de amónio (NH4VO3, um sal pentavalente de vanádio, na viabilidade celular e nas actividades enzimáticas GR, CAT T e CAT A das leveduras vínicas Saccharomyces cerevisiae UE-ME3 e Saccharomyces chevalieri UE-ME1. Os resultados obtidos mostram que S. chevalieri UE-ME1 revelou menor tolerância ao NH4VO3 do que S. cerevisiae UE-ME3, uma vez que culturas de S. chevalieri não sobreviveram para valores de concentração do sal de vanádio superiores a 7,5 mM enquanto que células de S. cerevisiae mantiveram-se viáveis em presença de metavanadato de amónio 75 mM.     Full Text Available OBJETIVOS: Sintetizar as estratégias para a obtenção de uma vacina contra rotavírus e papilomavírus humano (HPV, enfatizando-se o estágio atual das investigações quanto à segurança, reatogenicidade, imunogenicidade e eficácia. FONTE DOS DADOS: Os artigos publicados de 1996 a 2006, particularmente ao longo dos últimos 5 anos, representaram a fonte primária (base de dados: MEDLINE, PubMed, NIH, EUA, conferindo-se destaque aos recentes estudos envolvendo uma vacina contendo rotavírus atenuado de origem humana e dois tipos de vacinas para HPV testadas em humanos: as profiláticas e as terapêuticas. SÍNTESE DOS DADOS: Em relação às vacinas contra rotavírus, o contexto de estratégias compreendeu desde as preparações jennerianas até o advento da vacina com rotavírus atenuado de origem humana. Duas vacinas despontam como eficazes e seguras: uma, pentavalente, envolvendo vírus de bovinos e do homem; outra, de rotavírus atenuado oriundo da espécie humana, sendo que esta é avaliada em mais de 70.000 crianças no mundo. As vacinas contra HPV profiláticas visam impedir a infecção por esses vírus, enquanto que as vacinas terapêuticas pretendem tratar o indivíduo já infectado ou até o portador de uma lesão causada por HPV. As pesquisas com vacinas profiláticas começaram há alguns anos nos EUA e, hoje, aproximadamente 25.000 mulheres, em vários países, participam de estudos sobre essa vacina, que até o momento tem se mostrado segura, bem tolerada, altamente imunogênica e eficaz contra os tipos de HPV que causam as verrugas genitais e 70% dos casos de câncer do colo do útero. CONCLUSÕES:Uma vacina eficaz e segura contra rotavírus resultará em impacto significativo quanto à doença grave por esse enteropatógeno. Um programa global de imunizações que consiga atingir 60 a 80% das crianças durante os próximos 10 anos pode reduzir em pelo menos 50% as hospitalizações e a mortalidade por rotavírus. Antecipa
