This report on the status of the Office of Environment's program for inactive uranium mill tailings sites is an analysis of the current status and a forecast of future activities of the Office of Environment. The termination date for receipt of information was September 30, 1980. Aerial radiological surveys and detailed ground radiological assessments of properties within the communities in the vicinity of the designated processing sites in Canonsburg, Pennsylvania, Salt Lake City, Utah, and Boise, Idaho led to the designation of an initial group of vicinity properties for remedial action. The potential health effects of the residual radioactive materials on or near these properties were estimated, and the Assistant Secretary for Environment recommended priorities for performing remedial action to the Department's Assistant Secretary for Nuclear Energy. In designating these properties and establishing recommended priorities for performing remedial action, the Office of Environment consulted with the Environmental Protection Agency, the Nuclear Regulatory Commission, representatives from the affected State and local governments, and individual property owners. After notifying the Governors of each of the affected States and the Navajo Nation of the Secretary of Energy's designation of processing sites within their areas of jurisdiction and establishment of remedial action priorities, a Sample Cooperative Agreement was developed by the Department in consultation with the Nuclear Regulatory Commission and provided to the affected States and the Navajo Nation for comments. During September 1980, a Cooperative Agreement with the Commonwealth of Pennsylvania for the designated Canonsburg processing site was executed by the Department. It is anticipated that a Cooperative Agreement between the State of Utah and the Department to perform remedial actions at the designated Salt Lake City site will be executed in the near future.

A general practitioner's office is an economic unit where task delegation is an essential component in improving the quality and performance of work. To classify the preferences of general practitioners regarding the delegation of medical-administrative tasks to assistant medical-social secretaries. Conjoint analysis was applied to a random sample of 175 general practitioners working in metropolitan France. Ten scenarios were constructed based on seven attributes: training for medical secretaries, logistical support during the consultation, delegation of management planning, medical records, accounting, maintenance, and taking initiative on the telephone. A factorial design was used to reduce the number of scenarios. Physicians' socio-demographic variables were collected. One hundred and three physicians responded and the analysis included 90 respondents respecting the transitivity of preferences hypothesis. Perceived difficulty was scored 2.8 out of 5. The high rates of respondents (59%; 95% CI [51.7-66.3]) and transitivity (87.5%; 95% CI [81.1-93.9]) showed physicians' interest in this topic. Delegation of tasks concerning management planning (OR=2.91; 95% CI [2.40-13.52]) and medical records (OR=1.88; 95% CI [1.56-2.27]) were the two most important attributes for physicians. The only variable for which the choice of a secretary was not taken into account was logistical support. This is a first study examining the choices of general practitioners concerning the delegation of tasks to assistants. These findings are helpful to better understand the determinants of practitioners' choices in delegating certain tasks or not. They reveal doctors' desire to limit their ancillary tasks in order to favor better use of time for "medical" tasks. They also expose interest for training medical secretaries and widening their field of competence, suggesting the emergence of a new professional occupation that could be called "medical assistant". In 1981, the Director of the Office of Nuclear Reactor Regulation (NRR) of the Nuclear Regulatory Commission (NRC) chartered a task force to assess the office program of technical assistance and to recommend improvements. The task force divided the technical assistance program into four areas, and the practices in each area were assessed through a series of surveys of staff, management, and contractor personnel. Background There is little data regarding the effect of tooth whitening on microhardness of white spot lesions. This study was conducted to investigate the effect of home-bleaching and laser-assisted in-office bleaching on microhardness of demineralized enamel. Material and Methods Forty bovine incisors were selected and immersed in a demineralizing solution for 12 weeks to induce white spot lesions. Enamel blocks were prepared and randomly assigned to two groups of 20 each. The first group underwent home bleaching with 15% carbamide peroxide which was applied for 8 hours a day over a period of 15 days. In the second group, in-office bleaching was performed by 40% hydrogen peroxide and powered by irradiation from an 810 nm gallium-aluminum-arsenide (GaAlAs) diode laser (CW, 2W). This process was performed for 3 sessions every seven days, in 15 days. The specimens were stored in Fusayama Meyer artificial saliva during the experiment. Surface microhardness was assessed before and after the bleaching therapies in both groups. Results Microhardness decreased significantly following both home bleaching and laser-assisted in-office bleaching (pTooth whitening through home bleaching or laser-assisted in-office bleaching can result in a significant reduction in microhardness of white spot lesions. Therefore, it is suggested to take protective measures on bleached demineralized enamel. PURPOSE The clinician-colleague relationship is a cornerstone of relationship-centered care (RCC); in small family medicine offices, the clinician–medical assistant (MA) relationship is especially important. We sought to better understand the relationship between MA roles and the clinician-MA relationship within the RCC framework. METHODS We conducted an ethnographic study of 5 small family medicine offices (having informed by clinicians’ roles in hiring and managing MAs and the social familiarity of MAs and clinicians. Within the RCC framework, these findings can be seen as previously undefined constraints and freedoms in what is known as the Complex Responsive Process of Relating between clinicians and MAs. CONCLUSIONS Improved understanding of clinician-MA relationships will allow a better appreciation of how clinicians and MAs function in family medicine teams. Our findings may assist small offices undergoing practice transformation and guide future research to improve the education, training, and use of MAs in the family medicine setting. PMID:24615311

The technology program areas covered are Solar Buildings Technology, Wind Energy Technology, Photovoltaic Energy Technology, Geothermal Energy Technology, Biofuels and Municipal Waste Technology, Solar Thermal Technology, Hydropower Energy Technology, Ocean Energy Technology, and Electric Energy Systems and Energy Storage. The analytical and managerial support provided to the office and staff of the Deputy Assistant Secretary for Renewable Energy enabled a comprehensive evaluation of program and policy alternatives, and the selection and execution of appropriate courses of action from amongst those alternatives. On December 9, 1995, the Secretary of Energy requested that the Department`s Inspector General (IG) conduct a thorough examination of all Secretarial foreign travel from 1993 to December 1995 to include the purpose of each trip, the activities of each Federal participant in each trip, the funding of each trip, and claims for reimbursements for expenses by Federal trip participants. The Secretary also requested that the review include an assessment of travel authorization, voucher, traveler reimbursement, and auditing systems employed by the Department to identify steps that could be taken to reduce errors and improve accounting oversight. Additionally, the Secretary requested that the Inspector General conduct a thorough examination of the establishment and filling of the Department`s Ombudsman position. The Office of Inspector General (OIG) initiated a review into these matters and assigned primary responsibility for the review to the Office of Inspections. The purpose of this inspection was to conduct a thorough examination of the 16 Secretarial foreign trips from June 1993 to December 1995. This report focuses on the four trade missions because of their extent and cost. We examined a number of Departmental management systems and processes involved in planning and executing the 16 foreign trips. To determine the actual cost of the 16 trips, it was necessary to determine who participated in the trips and to identify the individual travel costs. We were required to perform extensive reviews of records and conduct a large number of interviews because the Department could not provide any specific documents that could accurately account for who actually participated on the 16 trips. Having identified who participated, it was then necessary to examine key aspects of the Department`s management systems. Our report contains 31 recommendations for corrective action.

Purpose: The diagnostic and therapeutic success of shoulder joint injection depends on its accuracy. Two recent studies reported high success (93–96%) for non-imaging-assisted anterior injection in anesthetized patients. This study examined the accuracy of anterior shoulder injection in awake patients under conditions similar to the office setting. Methods: The study group consisted of 166 consecutive patients with shoulder pain who underwent diagnostic magnetic resonance arthrography at a tertiary medical center in 2011–2012. As part of the examination, contrast material was injected into the glenohumeral joint via an anterior approach by a certified musculoskeletal radiologist without any image assistance. Success was defined as contrast material limited to the intra-articular joint on the scan, without scatter to the periauricular tissue. Results: A successful injection was verified by magnetic resonance arthography in 163 patients (98.2%). Conclusions: Non-assisted anterior shoulder injection is highly accurate (98.2%) in the hands of an experienced radiologist, even in awake patients.

Full Text Available The aim of this proof of concept study is to describe an ultrasound (US assisted simplified surgical procedure for pre-terminal great saphenous vein (GSV high ligation/division avoiding groin dissection and tributary interruption, in an office setting, in association to varices phlebectomy and saphenous vein foam occlusion treatment. Inclusion criteria: primary GSV reflux due to terminal valve, vein diameter >6 mm. By ultrasonography in standing position, the point GSV passing over the adductor longus muscle (about 3 cm from the junction is identified. This E (easy point, relatively superficial, free from tributaries and other structures, allows an easy grasping and extraction of the GSV vein through a 3 mm stab incision provided an ultrasonography assistance. The vein is divided/ligated about 2 cm distal from the ostium, the distal stump is cannulated and foam is injected on the distal segment from the E-point incision in a retrograde fashion, varices are avulsed by phlebectomy. Twenty procedures in 18 patients (venous clinical severity score: mean 3.15 - GSV diameter: mean 7.34 were performed, all the cases without inconveniences, with a duration not exceeding 10 min in addition to the phlebectomy procedure time. No complications as hemorrhage, infection, nerve lesion, lymphatic leak or thrombosis have been registered. At one month the residual saphenous stump length was in average 2.16 cm with complete closure of GSV in all. Three patients have been controlled at 6 months showing GSV complete closure. The procedure described is a simple office US assisted method for GSV ligationdivision, leaving the 2 last cm of the saphenofemoral junction. It could be associated to most of the procedures in use with limited additional time and resources required.

This project was initiated in FY 1984 to provide technical assistance to the Department of Energy (DOE), Office of Operational Safety (OOS) in developing and implementing its Assurance Program for Remedial Action (APRA), i.e., overview of the DOE remedial action programs. During this second year of the project,* the technical assistance included report and procedure reviews, and assistance with conducting the Uranium Mill Tailings Remedial Action Program (UMTRAP) Office (DOE/AL) appraisal. This included participation in preappraisal visits to UMTRAP sites in Canonsburg, Pennsylvania; Grand Junction, Colorado; and Salt Lake City, Utah. Pacific Northwest Laboratory (PNL) also transferred the PNL-developed document review software to the Oak Ridge Associated Universities (ORAU) staff in Grand Junction, Colorado, in anticipation of future document reviews by the ORAU staff. Other accomplishments have included publication of two formal documents and three project reports, preparation and presentation of five topical reports at national and international meetings, two foreign trip reports, and comments on proposed draft standards of the Environmental Protection Agency (40 CFR 193). The project manager has also participated on National Council on Radiation Protection and Measurements (NCRP) and American Society for Testing and Materials (ASTM) subcommittees developing decommissioning standards, as well as International Atomic Energy Agency (IAEA) advisory groups developing environmental monitoring guidelines

Bleaching might affect structural properties of composite materials, and lead to monomer release. This study aimed to evaluate the effect of Laser-assisted and conventional in-office bleaching on the release of BIS-GMA, TEGDMA, and UDMA monomers from a nanohybrid and a microhybrid BIS-GMA based composite. 32 samples of each composite, were divided into 4 subgroups; subgroup 1: Conventional in-office bleaching (CIB) with the Opalescence Boost PF 38% gel, subgroup 2: Laser-assisted bleaching (LBO) with the Opalescence Boost PF 38% gel, subgroup 3: Laser-assisted bleaching (LBH) with the JW Power bleaching gel, subgroup 4: (CO) control without bleaching. All the samples were immersed in tubes of 2cc Ethanol 75% medium. The released monomers were analyzed using the high performance liquid chromatography (HPLC) method 24 h, 7, and 28 days. Data's were analyzed by Univariate Analysis of Variance test followed by Tukeys HSD. The amount of TEGDMA monomer released was not significant. However, nanohybrid composites showed significantly more monomer release than microhybrid composites (P Bleaching by laser with JW Power Bleaching gel led to more monomer release in nanohybrid composite.

Aims: This study investigated the effectiveness of laser-assisted in-office bleaching and home-bleaching in sound and demineralized enamel. Materials and Methods: The sample consisted of 120 freshly-extracted bovine incisors. Half of the specimens were stored in a demineralizing solution to induce white spot lesions. Following exposure to a tea solution for 7.5 days, the specimens were randomly assigned to 4 groups of 30 according to the type of enamel and the bleaching procedure employed. Groups 1 and 2 consisted of demineralized teeth subjected to in-office bleaching and home bleaching, whereas in groups 3 and 4, sound teeth were subjected to in-office and home bleaching, respectively. A diode laser (810 nm, 2 W, continuous wave, four times for 15 seconds each) was employed for assisting the in-office process. The color of the specimens was measured before (T1) and after (T2) staining and during (T3) and after (T4) the bleaching procedures using a spectrophotometer. The color change (ΔE) between different treatments stages was compared among the groups. Results: There were significant differences in the color change between T2 and T3 (ΔE T2–T3) and T2 and T4 (ΔE T2–T4) stages among the study groups (pbleaching (group 1) as compared to the other groups (Pbleaching could provide faster and greater whitening effect than home bleaching on stained demineralized enamel, but both procedures produced comparable results on sound teeth. Full Text Available Executive Secretary who longs to stand in organizations should seek to meet market requirements, with primary knowledge about information and communication technology. Its attributions, proper to modern management standards, must be carried out so that this professional can assist managers efficiently. In order to do so, Executive Secretary should make use of tools that aid in speeding up their actions, becoming more productive. In this context, this research was carried out with the Executive Secretaries and students who already work in the area or who are in trainning programs, aiming to investigate the use of Information and Communication Technology tools in the Executive Secretary work. Data collection was done through a structured questionnaire with objective questions, applied on Campus IV/UFPB, whose data were analyzed quantitatively, with results presented in tables and graphs. Based on the results, it was possible to consider that most respondents use ICT tools in their secretarial assignments, such as the computer and the internet. It was found that the majority believes that the use of these tools helps in managing their time when it comes to their duties. Among those who do not make use of ICT tools, most claimed that the organizations in which they work do not have these tools.

A computer-assisted enzyme-linked immunosorbent assay (ELISA) system, based on kinetics of the reaction between substrate and enzyme molecules, was developed for testing large numbers of sera in laboratory applications. Systematic and random errors associated with conventional ELISA technique were identified leading to results formulated on a statistically validated, objective, and standardized basis. In a parallel development, an inexpensive system for field and veterinary office applications contained many of the qualities of the computer-assisted ELISA. This system uses a fluorogenic indicator (rather than the enzyme-substrate interaction) in a rapid test (15 to 20 minutes' duration) which promises broad application in serodiagnosis.

The NASA Glenn Office of Equal Opportunity Programs works to provide quality service for all programs and/or to assist the Center in becoming a model workplace. During the summer of 2004, I worked with Deborah Cotleur along with other staff members to create and modify customer satisfaction surveys. This office aims to assist in developing a model workplace by providing functions as a change agent to the center by serving as an advisor to management to ensure equity throughout the Center. In addition, the office serves as a mediator for the Center in addressing issues and concerns. Lastly, the office provides assistance to employees to enable attainment of personal and organizational goals. The Office of Equal Opportunities is a staff office which reports and provides advice to the Center Director and Executive Leadership, implements laws, regulations, and presidential executive orders, and provides center wide leadership and assistance to NASA GRC employees. Some of the major responsibilities of the office include working with the discrimination complaints program, special emphasis programs (advisory groups), management support, monitoring and evaluation, contract compliance, and community outreach. During my internship in this office, my main objective was to create four customer satisfaction surveys based on EO retreats, EO observances, EO advisory boards, and EO mediation/counseling. I created these surveys after conducting research on past events and surveys as well as similar survey research created and conducted by other NASA centers, program for EO Advisory group members, leadership training sessions for supervisors, preventing sexual harassment training sessions, and observance events. Assistant environmental health officers (AEHO) are health care providers (HCPs) who act as enforcers, educators and trusted role models for the public. This is the first study to explore smoking behaviour and attitudes toward tobacco control among future HCPs. Almost 30% of AEHO trainees did not know the role of AEHOs in counselling smokers to stop smoking, but 91% agreed they should not smoke before advising others not to do so. The majority agreed that tobacco control regulations may be used as a means of reducing the prevalence of smoking. Future AEHOs had positive attitudes toward tobacco regulations but lacked understanding of their responsibility in tobacco control measures.

The Assistant Secretary for Special Education and Rehabilitative Services announces a priority under the Rehabilitation Training program to establish a Job-Driven Vocational Rehabilitation Technical Assistance Center (JDVRTAC). The Assistant Secretary may use this priority for competitions in fiscal year (FY) 2014 and later years. We take this action to focus on training in an area of national need. Specifically, this priority responds to the Presidential Memorandum to Federal agencies directing them to take action to address job-driven training for the Nation's workers. The JDVRTAC will provide technical assistance (TA) to State vocational rehabilitation (VR) agencies to help them develop for individuals with disabilities training and employment opportunities that meet the needs of today's employers.

The purpose of this in vitro study was to compare the effectiveness of a novel Er,Cr:YSGG laser-assisted in-office tooth bleaching method with a conventional method by spectrophotometric analysis of the tooth color change. Furthermore, the influence of the application time of the bleaching gel on the effectiveness of the methods and the maintenance of the results 7 days and 1 month after the treatments were also evaluated. Twenty-four bovine incisors were stained and randomly distributed into four groups. Group 1 specimens received an in-office bleaching treatment with 35% H 2 O 2 for 2 × 15 min. Group 2 specimens received the same treatment but with extended application time (2 × 20 min). In Group 3, the same in-office bleaching procedure (2 × 15 min) was carried out as that in Group 1, using Er,Cr:YSGG laser irradiation for 2 × 15 s on each specimen to catalyze the reaction of H 2 O 2 breakdown. Group 4 specimens received the same bleaching treatment as Group 3 but with extended application time (2 × 20 min). Er,Cr:YSGG laser-assisted tooth bleaching treatment is more effective than the conventional treatment regarding color change of the teeth. Application time of the bleaching agent may influence the effectiveness of the methods. The color change of the tested treatments decreases after 7 days and 1 month. The clinical relevance of this study is that this novel laser-assisted bleaching treatment may be more advantageous in color change and application time compared to the conventional bleaching treatment.

For the defence against the threats through radioactive substances, a general concept is presently being elaborated under the overall control of the Federal Government. A number of competent organisations are involved in this, for example the Federal Office of Criminal Investigation, the Federal Armed Forces, and the Federal Office for Radiation Protection. In Germany, the 16 Federal States are responsible for the prevention of nuclear hazards. In the case of hazards through radioactive material, experts from the competent radiation protection authorities are consulted. For serious cases of prevention of nuclear hazards (nuclear fuels, criticality, danger of dispersion), the Federal Office for Radiation Protection - a subordinate authority of the Federal Ministry for the Environment, Nature Conservation and Nuclear Safety responsible for radiation protection - was given order to elaborate a concept for assistance to those Federal States. The goal of the Environmental Protection Agency's (EPA) Office of Radiation and Indoor Air is to protect the public and the environment from exposures to radiation and indoor air pollutants. The Office develops protection criteria, standards, and policies and works with other programs within EPA and other agencies to control radiation and indoor air pollution exposures; provides technical assistance to states through EPA's regional offices and other agencies having radiation and indoor air protection programs; directs an environmental radiation monitoring program; responds to radiological emergencies; and evaluates and assesses the overall risk and impact of radiation and indoor air pollution. The Office is EPA's lead office for intra- and interagency activities coordinated through the Committee for Indoor Air Quality. It coordinates with and assists the Office of Enforcement in enforcement activities where EPA has jurisdiction. The Office disseminates information and works with state and local governments, industry and professional groups, and citizens to promote actions to reduce exposures to harmful levels of radiation and indoor air pollutants

This report documents the results of the Line Program Environmental Management Audit completed for the Formerly Utilized Sites Remedial Action Program (FUSRAP). During this Audit, activities and records were reviewed and personnel interviewed at Oak Ridge, Tennessee. Additionally, since FUSRAP falls under the responsibility of the Office of the Assistant Secretary for Environmental Restoration and Waste Management, selected individuals from this office were interviewed in Washington, DC and Germantown, Maryland. The onsite portion of the FUSRAP Audit was conducted from March 16 through 27, 1992, by the US Department of Energy's Office of Environmental Audit (EH-24) located within the Office of the Assistant Secretary for Environment, Safety and Health (EH-1). The scope of the FUSRAP Line Program Environmental Management Audit was comprehensive and included all areas of environmental management with the exception of the National Environmental Policy Act (NEPA).
