    In 1994, the rural districts of Darmstadt-Dieburg, Bergstrasse, Odenwald and Gross-Gerau in southern Hessen and the town of Darmstadt concluded a cooperation agreement permitting them to meet the future demands of the technical code on municipal waste and the `Kreislaufwirtschafts- und Abfallgesetz` (act concerning waste recycling and waste management). In order to formally prepare different forms of cooperation and to accompany corresponding preliminary investigations, the Suedhessische Arbeitsgemeinschaft Abfallwirtschaft (SAGA) was founded as a municipal task force. SAGA`s task is to debate common affairs in the waste management sector with a view to joint planning and joint facilities. In particular, the requirements of the technical code on municipal waste must be met. (orig.) [Deutsch] Im Jahre 1994 haben die suedhessischen Landkreise Darmstadt-Dieburg, Bergstrasse, Odenwald, Gross-Gerau sowie die Stadt Darmstadt beschlossen, bezueglich der kuenftigen Anforderung der TA Siedlungsabfall und des Kreislaufwirtschafts- und Abfallgesetzes zu kooperieren. Zur formellen Vorbereitung von verschiedenen dann auszufuehrenden Kooperationsformen und zur Begleitung entsprechender Voruntersuchungen wurde die Suedhessische Arbeitsgemeinschaft Abfallwirtschaft (SAGA) als Kommunale Arbeitsgemeinschaft gegruendet. Aufgabe der Kommunalen Arbeitsgemeinschaft ist es, gemeinsam beruehrende Angelegenheiten im Abfallbereich unter dem Gesichtspunkt gemeinsamer Planung und gemeinsamer Einrichtungen zu beraten.     This annual report focuses on Rhine water quality from the view of water treatment for drinking. While the 'classic' parameters, i.e. nutrients, salts, heavy metals, summary organic substance parameters have reached have reached a more or less satisfactory and stable level, there are always enhanced concentrations or new compounds of anthropogenic zenobiotics. Jahresbericht vor, in dem ueber ihre Taetigkeiten, Ergebnisse aus dem Untersuchungsprogramm der Rhein-Wasserwerke and aktuelle Fragestellungen berichtet wird. In diesem Jahrsbericht wird wiederum schwerpunktmaessig auf die Beschaffenheit des Rheinwassers unter dem Aspekt der Trinkwassergewinnung eingegangen. Waehrend bei den klassischen Parametern (Naehrstoffe, Salze, Schwermetalle, summarische organische Messgroessen) inzwischen ein ueberwiegend guter chemischer Zustand erreicht ist, werden bei den anthropogenen xenobiotischen Einzelstoffen immer wieder erhoehte Konzentrationen oder 'neue' Verbindungen im Rhein entdeckt.     This is the 57th annual report of the Arbeitsgemeinschaft Rhein-Wasserwerke e.V. (ARW) on Rhine water quality from the view of drinking water supply. The positive trends continued in some fields, especially in the case of chloride concentrations and freights. On the other hand, there was a significant increase of some organic pollutants like DTPA, glyphosate and AMPA. The microbiological specifications of the new Drinking Water Ordinance and the concentrations of tensides and their metabolites are gone into. The ARW is worried about new or newly identified organic substances in Rhine water, especially as there is a lack of toxicological data on these substances and of data concerning their behaviour during freshwater preparation for drinking. [German] Die Arbeitsgemeinschaft Rhein-Wasserwerke e.V. (ARW) legt hiermit ihren nunmehr 57. Jahresbericht vor, in dem ueber ihre Aktivitaeten und wichtige Ergebnisse aus dem Untersuchungsprogramm der Rhein-Wasserwerke berichtet wird. Wie in den Vorjahren steht im Mittelpunkt der fachlichen Arbeiten der ARW die Bewertung der Rheinwasserbeschaffenheit aus Sicht der Trinkwasserversorgung. Im ersten Teil dieses Jahresberichtes sind die wichtigsten Ergebnisse zusammengefasst, die insgesamt erkennen lassen, dass sich die positiven Tendenzen bei vielen Wassergueteparametern auch im Berichtsjahr 2000 fortgesetzt haben. Dies gilt vor allem fuer die Chlorid-Konzentrationen und -Frachten im Rhein, die im Vergleich zum Vorjahr deutlich zurueckgegangen sind. Im Gegensatz dazu haben die Konzentrationen einiger organischer Einzelstoffe, wie z. B. DTPA sowie Glyphosat und AMPA, im Jahr 2000 z. T. deutlich zugenommen.     The group of biologics for the treatment of rheumatic diseases is continuously growing. They have become an important option not only for treatment of so far untreatable chronic inflammatory or rheumatic disease, but also for juvenile idiopathic arthritis. In addition, the velocity and the degree of improvement is better than with to conventional therapies. Furthermore, toxicity and risks seem to be lower with higher safety and compatibility. Although the data are scarce, they are widely used. Therefore, the German Arbeitsgemeinschaft Kinder- und Jugendrheumatologie is updating the current recommendation for the treatment of juvenile idiopathic arthritis using biologics.

    The ARGE Weser (Arbeitsgemeinschaft zur Reinhaltung der Weser), which comprises representatives of the German states of Bremen, Hessen, Niedersachsen, Nordrhein-Westfalen and Thueringen, publishes a yearly water quality report based on the measuring programmes for the Weser river and for the Werra/Ulster rivers (the latter is a joint project of the states of Hessen and Thrueringen). The Weser river today has a water quality of II-III after the successful implementation of the 1989 action programme. Chloride concentrations were reduced as well, but efforts will still be made by communal authorities, agriculture, industry and administrative bodies if the goals set for 2000 are to be reached. (orig/AJ) [Deutsch] Die Arbeitsgemeinschaft zur Reinhaltung der Weser (ARGE Weser), der die Bundeslaender Bremen, Hessen, Niedersachsen, Nordrhein-Westfalen und Thueringen angehoeren, dokumentiert jaehrlich im Weserguetebericht die Ergebnisse des Weser- und des von Hessen und Thueringen gemeinsam gefuehrten Messprogrammes Werra/Ulster. Die Weser weist heute ueberwiegend die Gewaesserguete II-III auf. Dies ist ein Ergebnis des 1989 beschlossenen Aktionsprogrammes. Deutliche Erfolge konnten auch im Hinblick auf die Reduzierung der Chloridbelastung erzielt werden. Die volle Verwirklichung des Aktionsprogrammes Weser bis zum Jahr 2000 wird allerdings noch erhebliche Anstrengungen auf Seiten der Kommunen, der Landwirtschaft, der Industrie und der Verwaltungen erfordern.     Work on the high-temperature gas turbine started in October 1986 in the context of AG Turbo. The 5th status seminar took a look back, with 19 papers on the fluid mechanics of turbomachinery, analysis and mathematical description of flow phenomena. (orig./AKF) [Deutsch] Vor 10 Jahren, im Oktober 1986, wurden die ersten Entwicklungsarbeiten an der Hochtemperatur Gasturbine im Rahmen der AG Turbo begonnen. Dieses Ergebnis veranlasst uns heute, am Beginn des 5. Statusseminars, zu einem Rueckblick mit 19 Beitraegen, die im vorliegenden Bericht enthalten sind. Turbotech befasst sich mit der Verbesserung der fluidmechanischen Energieumsetzung in Turbomaschinen. Hier wurden schwerpunktmaessig Fragen behandelt, die sich mit der Analyse und mathematischen Beschreibung der realen Stroemung hochbelasteter Turbinen und Verdichter befassen. Die Beherrschung der hohen Temperaturen, insbesondere vor der Turbine, steht im Mittelpunkt des Interesses von Turbotherm. Fuer die Turbine konnten neue Herstellungstechnologien und erstmals keramische Werkstoffe auf ihre Eignung zum Einsatz in dieser untersucht werden. Aufgabe von Turboflam ist die Entwicklung moderner hochbelasteter Brennkammern mit einer stabilen, schadstoffarmen Verbrennung fuer fluessige und gasfoermige Brennstoffe. Zum Erreichen des Entwicklungszieles wurden die beiden Arbeitsschwerpunkte Brennkammermodellierung und Entwicklung neuer Brennkammerkonzepte gebildet. (orig./AKF)

    The purpose of the meeting was to provide a forum for exchange of information on safety and licensing aspects for gas-cooled reactors in order to provide comprehensive review of the present status and of directions for future applications and development. Contributions were made concerning the operating experience of the Fort St. Vrain (FSV) HTGR Power Plant in the United States of America, the experimental power station Arbeitsgemeinschaft Versuchsreaktor (AVR) in the Federal Republic of Germany, and the CO/sub 2/-cooled reactors in the United Kingdom such as Hunterson B and Hinkley Point B. The experience gained at each of these reactors has proved the high safety potential of Gas-cooled Reactor Power Plants.

    Reviews were conducted at Kernforschungsanlage (KFA) Juelich of the US and Federal Republic of Germany (FRG) high-temperature gas-cooled reactor (HTGR) programs under the US/FRG Umbrella Agreement, with emphasis on those technology development areas where cooperation is ongoing and planned. Specific subprogram areas are safety; materials; fuels, fission products, and graphite; and Arbeitsgemeinschaft Versuchs-Reaktor (AVR). The purpose was to assess the status of the cooperation, reach agreement on any changes needed, and identify new areas of cooperation. Overall, the agreement has been both effective and beneficial. Ongoing activities complement and support US technology development plans. Discussions were held in the United Kingdom (UK) at the Risley Nuclear Power Development Laboratory regarding a potential graphite technology exchange program between the US Department of Energy and the UK Atomic Energy Authority. A draft agreement was reviewed and appeared to be satisfactory to both parties and ready for signature. A summary of potential areas of activity in the exchange had been prepared by US representatives and was discussed and found to be acceptable to UK representatives.

    The present report contains results on the occurrence of polar and nonpolar substances in the Elbe and selected tributaries which were analysed in the period from 1994 to 1999. It shows that industrial and municipal purification plants still have an insufficient cleaning capacity for certain substances. The process by which ubiquitous pollutants can enter the Elbe via a small, widely ramified fluvial system is exemplified for the family of chloroalkyl phosphates. These mainly occur in flame retardants and plasticisers in polyurethanes. The paper also presents results from a selection of 28 substances listed in Directive 76/464/ECC. They provide a decision aid for the planning of future measuring concepts based on the EU Water Framework Directive. In the ARGE ELBE programme organic pollutants are recorded and published on a routine basis. These are mostly nonpolar substances such as the well-known polychlorinated biphenyls or slow-evaporating insecticidal chlorinated hydrocarbons such as DTT. Recent analysis results show that this group of substances is relevant now as before and that it will remain so for the years to come. Special studies involving aqueous phase measurements were performed on a different group of substances, namely polar substances, which are generally readily soluble. The substances on which this report is chiefly focused originate from different sources: industrial and municipal effluents, diffuse inputs from agriculture and atmospheric depositions. In many cases they are difficultly degradable and could attain significance where river water in the form of river bank filtrate is used for drinking water production. However, due to complex and effective purification technologies they rarely enter drinking water. [German] Der vorliegende Bericht enthaelt Ergebnisse zum Vorkommen polarer und unpolarer Stoffe in der Elbe und in ausgewaehlten Nebenfluessen, die in dem Zeitraum von 1994 bis 1999 analysiert worden sind. Er zeigt, dass fuer einzelne Stoffe noch Defizite im Hinblick auf die Reinigungsleistung industrieller und kommunaler Klaeranlagen existieren. Wie ubiquitaer vorhandene Schadstoffe ueber ein kleines und weitverzweigtes Fliesssystem in die Elbe gelangen koennen, wird beispielhaft an der Stoffklasse der Chloralkylphosphate aufgezeigt. Diese finden hauptsaechlich Verwendung als Flammschutzmittel und Weichmacher in Polyurethanen. Ergebnisse einer Auswahl von 28 Stoffen aus der Richtlinie 76/464/EWG werden ebenfalls vorgestellt. Sie sind eine Entscheidungshilfe fuer die Planung zukuenftiger Messkonzepte auf der Grundlage der EU-Wasserrahmenrichtlinie. Mit dem Messprogramm der ARGE ELBE werden organische Schadstoffe routinemaessig erfasst und veroeffentlicht. Dabei handelt es sich ueberwiegend um unpolare Stoffe wie die hinlaenglich bekannten polychlorierten Biphenyle oder die schwerfluechtigen chlorierten Kohlenwasserstoffe mit insektizider Wirkung (z.B. DDT). Neuere Analysenergebnisse zeigen, dass diese Stoffgruppe nach wie vor relevant ist und auch in den kommenden Jahren nicht an Bedeutung verlieren wird. Eine andere Stoffgruppe, die polaren und im allgemeinen gut wasserloeslichen Substanzen, werden im Rahmen von Sonderuntersuchungen in der waessrigen Phase bestimmt. Die in diesem Bericht hauptsaechlich behandelten Stoffe stammen aus unterschiedlichen Quellen: industrielle und kommunale Abwaesser, diffuse Eintraege aus der Agrarwirtschaft und aus atmosphaerischen Depositionen. In vielen Faellen sind sie biologisch schwer abbaubar und koennen Bedeutung erlangen, wenn Flusswasser als Uferfiltrat fuer die Trinkwasserherstellung genutzt wird. Wegen der aufwendigen und wirkungsvollen Aufbereitungstechnologie gelangen sie allerdings nur selten bis ins Trinkwasser. (orig.)

    In 1979-1980, breams caught between Schnackenburg and Glueckstadt were analyzed for heavy metals and chlorinated hydrocarbons. After the German unification, the analyses were extended to further sections of the river. The report presents the results. [German] In den Jahren 1979-1980 wurden Brassen zwischen Schnackenburg und Glueckstadt fuer eine gezielte Analyse auf verschiedene Schwermetalle und Chlorkohlenwasserstoffe gefangen. Vorrangiges Ziel der damaligen ARGE-ELBE-Untersuchung war die Eignung des Weissfisches als Bioindikator zur Darstellung der Schadstoffbelastung in der Elbe. Nach der Wiedervereinigung wurde ein weiteres Sondermessprogramm mit dem Brassen durchgefuehrt. Der Brassen wurde an 10 Fangplaetzen im bundesdeutschen Elbelaengsschnitt 1994 befischt: An zwei Stationen in der Oberen Elbe, an drei Fangstellen in der Mittelelbe und an fuenf Fangorten in der Tideelbe. Die Befunde wurden hinsichtlich regionaler Belastung und Vermarktungsfaehigkeit dargestellt. In dem vorliegenden Bericht werden ausnahmslos die Schadstoffgehalte der Brassenfaenge der Elbe des Jahres 1994 ausgewertet. Zur Festlegung der Hintergrundbelastung (Referenzstandard) des Bewertungssystems wurden Brassen aus voraussichtlich gering belasteten Gewaessern beruecksichtigt. Ziel der Datenauswertung ist eine Klassifizierung, welche die regionalspezifische Kontamination im Muskelfleisch der Elbe-Brassen widerspiegelt. (orig.)

    This special report focuses on measurements of elements whose scope goes far beyond the routine monitoring for metals. The present studies are intended to extend the existing knowledge on the pollution situation of the Elbe and certain of its tributaries as well as provide a basis for formulating future guidelines for action. The first step towards this goal is sampling by means of an helicopter. The sampling is followed by a very involved and costly analysis programme, accompanied by extensive quality assurance measures. This is the only way of reliably analysing a large number of elements occurring in extremely low concentrations. Once the data have been recorded the question arises how they are to be evaluated. What schemes are suitable for categorising a large number of elements measured in either the suspended matter or the water compartment? What elements are to be regarded as the result of anthropogenic influences and what are their respective background concentrations in the fluvial system under study? These questions are approached by means of statistical methods which permit identifying elemental patterns and pollution sources as well as optimising the selection of measuring points for future studies. [German] In diesem Sonderbericht steht die Bestimmung von Elementen, die in ihrem Umfang ueber die routinemaessig erfassten Metalle deutlich hinausgeht, im Vordergrund. Die vorliegenden Untersuchungen sollen das bereits vorhandene Wissen zur Beschreibung der Immissionssituation der Elbe und von bestimmten Nebenfluessen erweitern, verbunden mit der Moeglichkeit, zukuenftige Handlungsmaximen zu erarbeiten. Zur Erreichung dieses Ziels steht zu Beginn die Probenahme mit einem Helicopter. Im Anschluss an die Probenahme folgt eine sehr aufwendige und teure Analytik, die begleitet wird von umfassenden Massnahmen zur Qualitaetssicherung. Nur so ist es moeglich, eine grosse Vielzahl von Elementen in extrem niedrigen Konzentrationen sicher analysieren zu koennen. Sind die Daten erhoben, schliesst sich die Frage nach ihrer Bewertung an. Welche Schemata sind tauglich, um eine Vielzahl von Elementen, die in den unterschiedlichen Kompartimenten Schwebstoff und Wasser bestimmt werden, einstufen zu koennen? Welche Elemente sind als anthropogen beeinflusst zu bezeichnen, welche Konzentrationen stellen den background in dem betrachteten Flusssystem vor? Zur Beantwortung dieser Fragen wird sich der Statistik bedient, denn mit ihrer Hilfe koennen Elementmuster identifiziert, Belastungsquellen aufgezeigt und darueber hinaus die Auswahl von Messstellen fuer kuenftige Untersuchungen optimiert werden.     Endocrineously active substances are discharged into surface bodies of water with communal and industrial effluents. Especially estrogenic substances like Bisphenol A, alkyl phenols and alkyl phenol ethoxylates in the Elbe and its tributaries have been neglected so far. In the context of an interdisciplinary project, chemists, biologists and mathematicians investigated the concentrations and fate of these xenoestrogens, relative potential effects of unknown water constituents and quantitative statistical mechanisms based on the available analytical findings. The contribution intends to contribute information on the pollutant situation of selected endocrineously active substances in the Elbe and its tributaries. [German] Ein relevanter Eintragspfad fuer endokrin wirksame Stoffe in die Oberflaechengewaesser sind die Abwaesser kommunaler und industrieller Klaeranlagen. Insbesondere das Vorkommen und der Verbleib von oestrogen wirkenden Stoffen wie Bisphenol A, den Alkylphenolen und Alkylphenolethoxylaten in der Elbe und in Elbenebenfluessen wurde bisher zu wenig Aufmerksamkeit geschenkt. In einem interdisziplinaeren Projekt, an dem sich Chemiker, Biologen und Mathematiker beteiligten, wird versucht, eine erste Antwort zu geben ueber das Vorkommen und den Verbleib dieser Xenooestrogene, ueber relative Wirkpotenzen von unbekannten Wasserinhaltsstoffen durch die Anwendung von In-vitro-Tests und ueber quantitative statistische Zusammenhaenge, die auf vorhandenen Analysenergebnissen basieren.     For more than three decades Germany had a very ambitious High Temperature Reactor (HTR) program which included numerous research activities and the construction and operation of two HTRs. The whole program had a volume of more than 6 billion D-Marks and covered activities of industry, research centres and universities. Within this program the physical feasibility of a pebble bed HTR was power for the first time in the Arbeitsgemeinschaft Versuchsreaktor (AVR) research reactor at Research Centre Julich (FZJ). Later it served as a test bed for new developed fuel as well as test reactor for numerous successful experiments on the inherent safety of this special type of nuclear reactor. The subsequent power plant THTR-300 with a rated electrical power of 300 MWel at Hamm-Uentrop was constructed as a demonstration plant. With THTR-300 the feasibility of a large commercial pebble bed reactor was demonstrated. Both reactors were built in the state of North-Rhine Westphalia with its traditional resources of coal and its heavy and chemical industry. HTRs were specifically meant to provide process heat to these industries and with the Project Nuclear Process Heat (PNP) a plant was developed to serve this means. Based on this HTR specific expertise and on actual experience from AP1000TM development and construction, Westinghouse has the overall expertise in house to design a generation IV reactor system in the near term future. On HTR specific systems and components the maturity of the technology was demonstrated with THTR-300. Potential design approaches for future HTR concepts for process heat generation are discussed.     Full Text Available [english] The Annual Meeting 2013 of the German MLA (AGMB e.V. took place from September 16 to 18 at the Charité – Universitätsmedizin Berlin. The conference was hosted by the Medical Library of the Charité.The main topics dealt with included evidence-based medicine and the role of librarians in the transfer of knowledge from research into practice, library services to support researchers in the publishing process, open access publishing, the role of libraries in research data management, pioneering models for the supply of digital natives with e-learning materials, as well as the requirements and tools of quality management in academic libraries. Work groups, meeting points and short presentations provided the participants with additional opportunities to gather information on current topics and to exchange their experiences. The programme was completed by a panel discussion on e-books, a workshop on systematic literature searching, an exhibition featuring the participating companies’ products and services, product reviews, a poster session and the prize giving event of the 2013 AGMB competition “Leuchttürme in Medizinbibliotheken”.

  1. Fuel-Cycle and Nuclear Material Disposition Issues Associated with High-Temperature Gas Reactors

    The objective of this paper is to facilitate a better understanding of the fuel-cycle and nuclear material disposition issues associated with high-temperature gas reactors (HTGRs). This paper reviews the nuclear fuel cycles supporting early and present day gas reactors, and identifies challenges for the advanced fuel cycles and waste management systems supporting the next generation of HTGRs, including the Very High Temperature Reactor, which is under development in the Generation IV Program. The earliest gas-cooled reactors were the carbon dioxide (CO2)-cooled reactors. Historical experience is available from over 1,000 reactor-years of operation from 52 electricity-generating, CO2-cooled reactor plants that were placed in operation worldwide. Following the CO2 reactor development, seven HTGR plants were built and operated. The HTGR came about from the combination of helium coolant and graphite moderator. Helium was used instead of air or CO2 as the coolant. The helium gas has a significant technical base due to the experience gained in the United States from the 40-MWe Peach Bottom and 330-MWe Fort St. Vrain reactors designed by General Atomics. Germany also built and operated the 15-MWe Arbeitsgemeinschaft Versuchsreaktor (AVR) and the 300-MWe Thorium High-Temperature Reactor (THTR) power plants. The AVR, THTR, Peach Bottom and Fort St. Vrain all used fuel containing thorium in various forms (i.e., carbides, oxides, thorium particles) and mixtures with highly enriched uranium. The operational experience gained from these early gas reactors can be applied to the next generation of nuclear power systems. HTGR systems are being developed in South Africa, China, Japan, the United States, and Russia.     The First Technical Report of the project 'Analysis of Seasonal Storage of Solar Heat in Mines' describes the local terms of construction and operation of a shaft storage at the 'Bergwerk General Blumenthal/Haard' in Recklinghausen. For the installation of a pilot storage the Shaft No. 6 should be chosen because of the better condition of the shaft walling and the greater diameter in comparison with Shaft No. 2. As the result of the objective of operational reliability only the leakproof metal tubbing array in the shaft should be used. In compliance with this rule, a system with a volume of 2,155 m{sup 3} is designated for the Shaft No. 6 (storage height 63 m). The storage is only insulated at the top. A gravel/water filling guarantees static strength of the site. With a vertical distance of 1.5 m, 42 pipe coils should be installed, evenly distributed across the cross section and connected to 10 temperature-dependent controllable layers. Each pipe coil consists of 100 m steel pipes with an inside diameter of 2.5 cm. Simulation calculations show that storage efficiencies higher than 80% can be expected. The efficiencies and the discharged energy considerably depend on the temperature level of the district heat netowrk. The collector area and the quantity of heat have a comparatively smaller influence on the storage operation results. The specific investment costs of the shaft storage at the location 'General Blumenthal 6' were calculated to 477 DM/m{sup 3}. A reduction of the specific costs can be achieved especially by the use of larger storage volumes in the future. (orig.) [German] Im vorliegenden Bericht werden die Rahmenbedingungen fuer den Bau und Betrieb eines Schachtspeichers am Beispiel des Standortes Bergwerk General Blumenthal/Haard in Recklinghausen konkretisiert. Da der Schacht 2 in einem deutlich schlechteren Zustand ist und einen kleineren Schachtdurchmesser aufweist, ist es sinnvoll, den Schacht 6 fuer die Installation eines Pilotspeichers zu nutzen. Aufgrund des Zieles einer moeglichst hohen Betriebssicherheit sollte fuer den Schachtspeicher der eigendichte Abschnitt mit Tuebbingausbau, der in diesem Fall bis 70 m Teufe reicht, verwendet werden. Abzueglich eines unteren Ueberlappungsbereiches und des Schachtkellers ergibt sich somit eine nutzbare Speicherhoehe von rund 63 m, was einem Volumen von etwa 2.155 m{sup 3} entspricht. Der Speicher wird nur am Kopf waermegedaemmt. Der Schachtspeicher wird mit einer Kies/Wasser-Fuellung versehen. In diese werden mit einem vertikalen Abstand von 1,5 m insgesamt 42 Rohrschlangenebenen eingebracht, die jeweils aus gleichmaessig ueber die Querschnittsflaeche verteilten Stahlrohren DN 25 mit einer Gesamtlaenge von knapp 100 m bestehen. Diese Ebenen werden zu insgesamt 10 temperaturabhaengig ansteuerbaren Schichten zusammengefasst. Durchgefuehrte Simulationsrechnungen zeigen, dass ein Grubenspeicher im quasistationaeren Zustand Nutzungsgrade von ueber 80% erreichen kann. Die Nutzungsgrade und damit die nutzbaren solaren Waermemengen haengen vor allem vom Temperaturniveau des Nahwaermenetzes ab. Die Groesse der Kollektorflaeche und die Hoehe der Waermelast der Verbraucher haben nur einen vergleichsweise geringen Einfluss auf die Leistungsfaehigkeit des Speichers. Bei positiver, aber realistischer Einschaetzung der Randbedingungen ergeben sich fuer den projektierten Pilotspeicher am Standort General Blumenthal 6 spezifische Investitionskosten in Hoehe von 477 DM/m{sup 3}. Eine Verringerung dieses Wertes ist vor allem durch die Vergroesserung des Speichervolumens bei zukuenftigen Anlagen moeglich. (orig.)

    Full Text Available Abstract Background The activity of the epidermal growth factor receptor (EGFR-directed monoclonal antibody cetuximab combined with oxaliplatin/leucovorin/5-fluorouracil (FUFOX was assessed in first-line metastatic gastric and oesophago-gastric junction (OGJ cancer in a prospective phase II study showing a promising objective tumour response rate of 65% and a low mutation frequency of KRAS (3%. The aim of the correlative tumour tissue studies was to investigate the relationship between EGFR gene copy numbers, activation of the EGFR pathway, expression and mutation of E-cadherin, V600E BRAF mutation and clinical outcome of patients with gastric and OGJ cancer treated with cetuximab combined with FUFOX. Methods Patients included in this correlative study (n = 39 were a subset of patients from the clinical phase II study. The association between EGFR gene copy number, activation of the EGFR pathway, abundance and mutation of E-cadherin which plays an important role in these disorders, BRAF mutation and clinical outcome of patients was studied. EGFR gene copy number was assessed by FISH. Expression of the phosphorylated forms of EGFR and its downstream effectors Akt and MAPK, in addition to E-cadherin was analysed by immunohistochemistry. The frequency of mutant V600E BRAF was evaluated by allele-specific PCR and the mutation profile of the E-cadherin gene CDH1 was examined by DHPLC followed by direct sequence analysis. Correlations with overall survival (OS, time to progression (TTP and overall response rate (ORR were assessed. Results Our study showed a significant association between increased EGFR gene copy number (≥ 4.0 and OS in gastric and OGJ cancer, indicating the possibility that patients may be selected for treatment on a genetic basis. Furthermore, a significant correlation was shown between activated EGFR and shorter TTP and ORR, but not between activated EGFR and OS. No V600E BRAF mutations were identified. On the other hand, an interesting trend between high E-cadherin expression levels and better OS was observed and two CDH1 exon 9 missense mutations (A408V and D402H were detected. Conclusion Our finding that increased EGFR gene copy numbers, activated EGFR and the E-cadherin status are potentially interesting biomarkers needs to be confirmed in larger randomized clinical trials.     Spain has to manage about 3700 tons of irradiated graphite from the reactor Vandellos I as radioactive waste. 2700 tons are the stack of the reactor and are still in the reactor core waiting for retrieval. The rest of the quantities, 1000 tons, are the graphite sleeves which have been already retrieved from the reactor. During operation the graphite sleeves were stored in a silo and during the dismantling stage a retrieval process was carried out separating the wires from the graphite, which were crushed and introduced into 220 cubic containers of 6 m{sup 3} each and placed in interim storage. The graphite is an intermediate level radioactive waste but it contains long lived radionuclides like {sup 14}C which disqualifies disposal at the low level waste repository of El Cabril. Therefore, a new project has been started in order to investigate two new options for the management of this waste type. The first one is based on a selective decontamination of {sup 14}C by thermal methods. This method is based on results obtained at the Research Centre Juelich (FZJ) in the Frame of the EC programs 'Raphael' and 'Carbowaste'. The process developed at FZJ is based on a preferential oxidation of {sup 14}C in comparison to the bulk {sup 12}C. Explanations for this effect are the inhomogeneous distribution and a weaker bounding of {sup 14}C which is not incorporated in the graphite lattice. However these investigations have only been performed with graphite from the high temperature reactor Arbeitsgemeinschaft Versuchsreaktor Juelich AVR which has been operated in a non-oxidising condition or research reactor graphite operated at room temperature. The reactor Vandellos I has been operated with CO{sub 2} as coolant and significant amounts of graphite have been already oxidised.     Presently axial compressors have to run with considerable distance to operation points of high load although usually the best efficiencies can be obtained there. In those areas there is high danger of surge which brings high damaging potential to the compressor. The prediction time to surge presently is very short (<<1s) and depends on the load and rotor rotation of the compressor. The goal of this project is the production of a data basis to investigate stall inception (which is the precursor of surge) and to develop and test better precursory warning algorithms. An extensive set of data as well in stable as in unstable operating points in different load areas was developed which now is ready for use in research, verification and for comparison of forewarning systems. [German] Zur Zeit ist es notwendig, Axialverdichter mit deutlichem Abstand von den hoch belasteten Betriebspunkten zu betreiben, obwohl haeufig dort die besten Wirkungsgrade zu erzielen waeren. In diesen Bereichen kann es leicht zu sog. Pumpen kommen, wodurch der Verdichter einem extremen Schaedigungspotential ausgesetzt ist. Die Vorwarnzeiten fuer das Pumpen sind bisher noch sehr kurz. Sie liegen in einem Bereich von deutlich weniger als einer Sekunde (abhaengig von der Belastung und der Drehzahl des Verdichters). Ziel dieser Untersuchungen ist es, eine Datenbasis zu liefern, anhand derer die Entstehungsmechanismen untersucht und moeglichst fruehzeitige Vorwarnalgorithmen entwickelt bzw. erprobt werden koennen. Es wurde eine umfangreiche Datenbasis sowohl an verschiedenen stabilen als auch instabilen Betriebspunkten in unterschiedlichen Lastbereichen erstellt, die nun zur Entwicklung, zur Verifizierung und zum Vergleich von Fruehwarnsystemen zur Verfuegung steht.     Full Text Available The annual meeting 2011 of the German MLA (AGMB took place 19th to 21th September at the university hospital of Cologne and was organized by the German National Library of Medicine. The elections of the new AGMB executive board and the new heads of the working teams were added to the fixed agenda with the commercial exhibition, scientific presentations, product reviews, meeting points and workshops. A new part of the meeting was the so called 5-minutes-presentation. Focal points were innovation management and marketing, reference management software and new applications in medical libraries.

    This (partial) project has the following aims: to describe comprehensively the possibilities for the recycling, or generation of energy from, waste with a high calorific value having undergone mechanical-biological conditioning; further, to formulate demands regarding the quality of the separated partial fractions. This basic study takes all relevant, commercial thermal processes into account (power plants, cement works, blast furnaces, etc.). Furthermore, the question is investigated of whether the thermal waste processing plants in the area of the Suedhessische Arbeitsgemeinschaft Abfall (SAGA) are suitable for waste utilization.     The Pebble Bed Modular Reactor (PBMR) is a First of a Kind Engineering with respect to the over 200 new innovations used in the design. The PBMR technical design is an inherited modified design from an earlier design such as the German 15 MWe AVR (Arbeitsgemeinschaft Versuchs Reaktor) and the THTR-300 MWe Thorium High Temperature Reactor (THTR), which ran in Germany as a test and research facility for 20 years. This paper discusses the Operator Training Simulator System for the PBMR Demonstration Power Plant. The Operator Training Simulator System will be used for operator training and licensing of plant operators. Included in the discussion is an overview of the major elements of the Operator Training Simulator System, including some of the main functional areas.

    Chronic obstructive pulmonary disease (COPD) is a leading cause of death worldwide. Cigarette smoking is the main cause of COPD. Quitting smoking is thus the most effective treatment strategy and central in COPD prevention. A number of guidelines on prevention, diagnosis, therapy and rehabilitation of COPD have been published. To help implementing and standardizing smoking cessation in COPD a guideline was published 2008 in Germany focusing on this complex issue. The present guideline is an update of the 2008 guideline and has a high grade of evidence (S3 according to the AWMF; Arbeitsgemeinschaft wissenschaftlicher medizinischer Fachgesellschaften). The guideline gives comprehensive and practical information on how to integrate smoking cessation as an central part of COPD therapy.

The printed journal version ceased. Thitherto it was sent to all members of the AGMB. The editorial staff made a few arrangements in order to support acceptance and perceptibility of this journal, now issued electronic only. The published papers were included in the database of the Deutsches Bibliothekswesen (DABI and in the Directory of Open Access Journals (DOAJ Content. Furthermore current articles were introduced in MEDINFO, a weblog for medical librarians. The paper compares usage data trends from September 2005 to May 2008 for selected articles.

    Full Text Available Despite major economic and organizational changes in the pharmaceutical industry, the Pharmaceutical Library Task Force (Arbeitskreis Pharma-Bibliotheken has been going strong in the Medical Library Working Party (Arbeitsgemeinschaft für medizinisches Bibliothekswesen – AGMB for the past 18 years. However, Task Force discussion topics have moved away from questions about spatial or staffing expansion or building up inventory, and toward themes such as outsourcing, training, technology scouting, and building up comprehensive copyright expertise. While actual physical stock is shrinking or has been eliminated entirely from many company libraries, the everyday working lives of company librarians are increasingly taken up with management of electronic media, in-house marketing and the associated user training, evaluation and use of new technologies, and clarification of complex copyright issues. Widespread know-how, in-depth knowledge of specialist areas (such as copyright, communication skills, networking, cost awareness, adaptability, willingness to learn new things, and demonstrating the value they add to the company are the factors that will secure the future of company librarians, whether with – or, in a pinch – without a physical library.

    In 2003, the program "The combined DAK therapy of obesity for children and adolescents", funded and conducted by the Deutsche Angestellten Krankenkasse (DAK) (A German Health Insurance Company), has started. The whole treatment lasts for 1 year including an initial inpatient therapy for 6 weeks followed by an outpatient treatment at home that adresses the overweight patients and their families. The therapy contents are developed according to the recommendations of the "Arbeitsgemeinschaft Adipositas im Kindes- und Jugendalter (AGA)". In a prospective cohort study a sample of 604 subjects was studied in order to examine the achievement of the treatment goals weight reduction, behaviour modification and improvement of quality of life. The development of weight was evaluated using BMI-SDS. 44,1% of children and adolescents had a successful weight reduction, they reduced their weight at least by 0,3 BMI-SDS. Furthermore, significant changes of health behaviour, physical fitness and quality of life were observed. However, during the outpatient treatment an impairment of some behaviour changes were observed. Nevertheless, the study has identified significant, positive effects in weight loss, behaviour modifications, changes in physical fitness and in the development of quality of life as a result of the therapy. It is demonstrated that a relapse in "old behaviour" followed by an increase of weight after the inpatient treatment can be avoided bythe subsequent outpatient therapy.

    In Germany, there is currently no guideline for pain assessment in elderly people. Pain management in nursing home residents is, however, legally required. For this particular group, especially for people with dementia, suitable interdisciplinary orientations for health care are lacking in Germany. The working group "Pain and Age" of the German Pain Society ("Deutschen Schmerzgesellschaft") in conjunction with the German Centre for Neurodegenerative Diseases ("Deutschen Zentrum für Neurodegenerative Erkrankungen"), Witten, has embarked on the development of interdisciplinary S3-Guideline for "Pain Assessment in Elderly People in Nursing Homes", based on the methodology suggested by the Association of the Scientific Medical Societies ("Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften e. V."), the German Agency for Quality in Medicine ("Ärztliche Zentrum für Qualität in der Medizin"), and that described in the DELBI ("Deutschen Leitlinien-Bewertungsinstrument"). Delegates of the 38 scientific societies and interest groups currently participating can contribute to the contents on three different levels. The present article outlines the methods for developing the guideline.

    In 1991 the North-Rhine Westphalian Government established the so-called Solar Working Group as a platform for the selective promotion of solar energy research and utilisation projects. In opting for the realisation of the ``high-flux density solar furnace`` project DLR has undertaken to provide an easily accessible, infrastructurally well-equipped installation for all experiments requiring highly concentrated sunlight. The project aim set out at the beginning has been realised to the full. The performance data of the solar furnace show that the original specifications have been overfulfilled in some cases. The great response and the very favourable operating results of the solar furnace since 1994 clearly demonstrate that this kind of plant can be operated satisfactorily under radiation conditions as they prevail in the region around Koeln. (AKF) [Deutsch] Im Jahre 1991 ist die Arbeitsgemeinschaft Solar durch die nordrheinwestfaelische Landesregierung ins Leben gerufen worden, um Projekte zur Erforschung und Nutzung von Solarenergie gezielt foerdern zu koennen. Um all denjenigen Experimenten, die hochkonzentrierte Sonnenstrahlung benoetigen, eine leicht erreichbare und mit guter Infrastruktur ausgestattete Anlage bieten zu koennen, hat sich die DLR zur Verwirklichung des Projektes `Hochflussdichte-Sonnenofen` entschieden. Das gesetzte Projektziel konnte in vollem Umfang erreicht werden. Die Leistungsdaten des Sonnenofens zeigen, dass die Auslegungswerte zum Teil uebertroffen werden konnten. Die grosse Resonanz und die sehr positiven Betriebsergebnisse des Sonnenofens seit Juni 1994 zeigen deutlich, dass der Betrieb einer solchen Anlage auch unter den Einstrahlungsbedingungen in der Koelner Region sinnvoll ist.     Georadar can be an important tool in the geophysical prospecting of lines considered for the laying of buried gas and water mains. But current devices as yet do not perform adequately because of the mostly very complex subsurface conditions. The Georadar Development Joint Venture, a task force of six German gas utilities and the American market leader in the georadar sector, has set itself the task of developing a new system specially for the prospecting of lines considered for the laying of mains. As field tests under real and artificial conditions of use show, definite progress has been made already during the first stage of the project with antenna and software development. (orig.)     Between October 1974 and October 1978 23 children with acute myelocytic leucemia (AML) received intensive therapy in the Univ.-Kinderklinik Münster: 4 children were treated according to the ALGB-protocol consisting of 5-7 day courses of ARA-C-infusion and 3 DNR-injections. 19 patients received the West-Berlin-protocol: The first 7 the original ALL protocol, 11 the modified form of AML, which will be presented here as AML-therapy-study BFM 78. 4 of the 23 patients died with early acute cerebral bleeding. 2 patients were nonresponders. 17 children went into remission. One girl died in remission of septicemic aspergillosis. 4 children had a relapse. In November 1978 there were still 12 patients in continuous complete remission, 3 of them already without therapy. 13 of the 19 patients, who were treated with the West-Berlin-protocol went into remission. 1 had a relapse. At present there are 11 patients in continuous complete remission. The above results and those found in the literature could signify that the long term prognosis of children with AML will be improved.     Full Text Available Appropriate identification and validation of biomarkers as well as pharmacogenetics are important in formulating patient-oriented, individualized chemotherapy or biological therapy in cancer patients. These markers can be especially valuable in pancreatic cancer, where high mortality and complex disease biology are frequently encountered. Recently, several advances have been made to further our knowledge in this specific area of pancreatic cancer. Elf TeilnehmerInnen waren bereit, im Anschluss an die Teilnahme am Kurs ihre Erkenntnisse in ein Gemeinschaftswerk einfließen zu lassen, dessen Entstehung in der digitalen Werkstatt koordiniert wurde (Adobe Connect und Moodle. Aus der Überzeugung heraus, dass das gemeinsame Besuchen eines solchen Online-Kurses nur ein Teil eines Lernerlebnisses sein kann und das gemeinsame Erarbeiten für die Teilnehmerinnen und Teilnehmer ein zusätzlicher Schritt zum Überblicken der schier unendlichen Angebotslandschaft darstellt. Diese diskutierten, entwickelten, verwarfen und evaluierten interessante Lernangebote.     This is a short report on the recent study by the Wuppertaler Institute (Executive Director: Prof. Dr. Ernst U. von Weizsaecker) under a mandate from the Swiss Working Group 'Solar 91' with the support of the Swiss Federal Office of Energy. The conclusion is that energy efficiency and a broad market penetration of renewable energy sources would reduce unemployment in Switzerland by a factor of 2 and create at least 63,000 new jobs. Background for the publication of this report at the end of August 1999 was the Swiss parliament debate on new energy tax regulations to be submitted to the poles in the course of 2000. The two houses of the parliament had different opinions on the matter.     Full Text Available Abstract Background Fractures of the proximal radius need to be classified in an appropriate and reproducible manner. The aim of this study was to assess the reliability of the three most widely used classification systems. Methods Elbow radiographs images of patients with proximal radius fractures were classified according to Mason, Morrey, and Arbeitsgemeinschaft für osteosynthesefragen/Association for the Study of Internal Fixation (AO/ASIF classifications by four observers with different experience with this subject to assess their intra- and inter-observer agreement. Each observer analyzed the images on three different occasions on a computer with numerical sequence randomly altered. Results We found that intra-observer agreement of Mason and Morrey classifications were satisfactory (κ = 0.582 and 0.554, respectively, while the AO/ASIF classification had poor intra-observer agreement (κ = 0.483. Inter-observer agreement was higher in the Mason (κ = 0.429-0.560 and Morrey (κ = 0.319-0.487 classifications than in the AO/ASIF classification (κ = 0.250-0.478, which showed poor reliability. Conclusion Inter- and intra-observer agreement of the Mason and Morey classifications showed overall satisfactory reliability when compared to the AO/ASIF system. The Mason classification is the most reliable system.

Quality inspection has to be integrated in the production sequence in order to ensure the conformance of the products. The interactions between manufacturing processes and inspections are very complex since three aspects (quality, cost, and time should all be considered at the same time while determining the suitable inspection strategy. Therefore, a simulation approach was introduced to solve this problem.The simulator called QUINTE [the QUINTE simulator has been developed at the University of Dortmund in the course of two research projects funded by the German Federal Ministry of Economics and Labour (BMWA: Bundesministerium für Wirtschaft und Arbeit, the Arbeitsgemeinschaft industrieller Forschungsvereinigungen (AiF, Cologne/Germany and the Forschungsgemeinschaft Qualität, Frankfurt a.M./Germany] was developed to simulate the machining as well as the inspection. It can be used to investigate and evaluate the inspection strategies in manufacturing processes.     Full Text Available The journal impact factor (IF, which is published annually by the Institute for Scientific Information® (USA, is meanwhile in widespread use as a scientometric parameter for the evaluation of research and researchers in Germany and other European countries. The present article subjects the IF to critical analysis. It first deals with processes of production, transfer, and use of medical knowledge, because the IF intervenes in these processes on account of its reflexivity. Secondary effects of the IF resulting from its reflexivity are discussed with the focus on the level of the author, the journal and the medical discipline as well as on social knowledge processes in society. In addition, the extent to which the IF is appropriate for evaluating the quality of a specific article, of a journal or of individual and collective research achievements is discussed.     The working group IGW/LWI (Ingenieurgemeinschaft Witzenhausen Fricke and Turk GmbH and Leichtweiss Institute of TU Brunswick University) carried out scientific and technical investigations of anaerobic digestion and subsequent composting at the mechanical-biological waste treatment plant at Kufstein/Tyrol on behalf of KOGAS GmbH. The investigations started in May 2000; they were to provide information for analysis and optimisation of the process and products with a view to the minimisation of residues for dumping or incineration. The following subjects were investigated: Mass balance; decomposition rate; gas yields and gas quality; quality of residues for dumping (biological stability and calorific value); off-gas emissions; process control. Performance data of the plant will provide a basis for further planning and will be interpreted against the background of current legal boundary conditions.     Full Text Available Die Einführung der elektronischen Gesundheitsakte, kurz ELGA, in Österreich ist ein seit längerer Zeit kontrovers diskutiertes Thema. Gerade in den letzten Monaten waren vermehrt Lebenszeichen der ELGA in der Presse zu finden, deren Umsetzung Teil der aktuellen Regierungsvereinbarung ist. Zudem wurden in vielen Gesprächen im Rahmen des Studiums immer wieder Vorwürfe geäußert, dass die Vorgaben seitens der Arbeitsgemeinschaft ELGA (ARGE ELGA zu sehr techniklastig seien, während die Forderungen der einzelnen zukünftigen Benutzer nicht oder zu wenig Beachtung finden. Im Rahmen der vorliegenden Arbeit wurde erarbeitet, wie sehr sich wirklich die Vorgaben seitens der ARGE ELGA auf den Bereich Technik konzentrieren bzw. welche Benutzeranforderungen bereits definiert wurden. Im nächsten Schritt wurden mittels eines problemzentrierten Interviews verschiedene Benutzergruppen der geplanten ELGA zu deren funktionalen Benutzeranforderungen befragt. Die ausgewählten Stakeholder waren: Ärzte, Apotheker, Soziale Dienste und Vertreter von Selbsthilfeorganisationen.Aus den durchgeführten 29 Interviews wurde im Anschluss eine Auswertung von funktionalen Benutzeranforderungen für die bislang beschriebenen Kernanwendungen der ELGA (Portal, e-Arztbrief, e-Befund Labor/Radiologie und e-Medikation erstellt. So wurde am Vordringlichsten die Einführung einer elektronischen Medikamentenübersicht bzw. der e-Impfpass gewünscht. Aber auch die Möglichkeit zur Präsentation der Selbsthilfe im Rahmen des ELGA-Portals war ein Punkt mit hoher Zustimmung. Demgegenüber wurde der Vorschlag des Versteckens von Dokumenten bzw. der Online-Kontakt mit dem behandelnden Arzt deutlich abgelehnt. Als Ergebnis wird zusammengefasst, dass derzeit keine ausreichende Definition von funktionalen Benutzeranforderungen für die elektronische Gesundheitsakte verfügbar ist.     The aim of this study was to compare the effects and indications of minimally invasive plate osteosynthesis (MIPO) and limited open reduction (LOR) for managing distal tibial shaft fractures. A total of 79 cases of distal tibial shaft fractures were treated surgically in our trauma center. The 79 fracture cases were classified into type A, B, and C (C1) according to the Arbeitsgemeinschaft für Osteosynthesefragen (AO) classification, with 28, 32, and 19 cases, respectively. Among the 79 fracture cases, 52 were closed fractures and 27 were open fractures (GUSTILO, I-II). After adequate preparation, 48 cases were treated with LOR and 31 cases were treated with MIPO. All cases were followed up for 12 to 18 months, with an average of 16.4 months. During the follow-up period, 76 fracture cases were healed in the first stage, whereas the 3 cases that developed non-union were treated by changing the fixation device and autografting. For types A, B, and some of C simple fractures (C1), LOR accelerated the fracture healing and lowered the non-union rate. One case suffered from regional soft tissue infection, which was controlled by wound dressing and intravenous antibiotics. Another case that developed local skin necrosis underwent local flap transplant. LOR promoted bone healing and lowered the non-union rate of several simple-distal tibial shaft fractures. Thereafter, the incidence of soft tissue complication was not significantly increased. However, for complex and comminuted fractures, MIPO was the preferred method for correcting bone alignment and protecting soft tissue, leading to functional recovery.

    The national goals of reducing CO{sub 2} emissions by 25% needs activities in cooperation with the German States, municipalities, business representatives and the private households. The working team developed a concept for a national climate protection campaign which addresses private households and the commercial sector. The campaign is to be implemented and financed jointly with the political and public administrations, and the business sector (public-private-partnership). Fundamental for the concept is a marketing strategy which both relies on ethical and participatory methods. Participatory marketing means the effort to include as many people as possible into the marketing of climate protection. The broad participation of representatives and supporters is not restricted to the implementation of climate-friendly measures, but should be achieved already in the planning stage and during evaluation of results.   2. The Role of Minimally Invasive Plate Osteosynthesis in Rib Fixation: A Review.

    More than a century ago, the first scientific report was published about fracture fixation with plates. During the 1950's, open reduction and plate fixation for fractures were standardized by the founders of Arbeitsgemeinschaft für osteosynthesefragen/Association for the Study of Internal Fixation. Since the introduction of plate fixation for fractures, several plates and screws have been developed, all with their own characteristics. To accomplice more fracture stability, it was thought the bigger the plate, the better. The counter side was a compromised blood supply of the bone, often resulting in bone necrosis and ultimately delayed or non-union. With the search and development of new materials and techniques for fracture fixation, less invasive procedures have become increasingly popular. This resulted in the minimally invasive plate osteosynthesis (MIPO) technique for fracture fixation. With the MIPO technique, procedures could be performed with smaller incisions and thus with less soft tissue damage and a better preserved blood supply. The last 5 years rib fixation has become increasingly popular, rising evidence has become available suggesting that surgical rib fixation improves outcome of patients with a flail chest or isolated rib fractures. Many surgical approaches for rib fixation have been described in the old literature, however, most of these techniques are obscure nowadays. Currently mostly large incisions with considerable surgical insult are used to stabilize rib fractures. We think that MIPO deserves a place in the surgical treatment of rib fractures. We present the aspects of diagnosis, preoperative planning and operative techniques in regard to MIPO rib fixation.

  3. Incidence of tendon entrapment and dislocation with calcaneus and pilon fractures on CT examination

    To examine the association between tibial pilon and calcaneal fracture classification and tendon entrapment or dislocation. After institutional review board approval, we retrospectively reviewed consecutive CT scans with calcaneal or pilon fractures from 5 years at a level 1 trauma center. We categorized calcaneal fractures according to the Sanders classification, and pilon fractures according to the Ruedi and Allgower and the Arbeitsgemeinschaft fuer Osteosynthesefragen-Orthopaedic Trauma Association (AO-OTA) classifications. Ankle tendons were assessed for dislocation or entrapment. Fisher's exact test was used for statistical analysis with significance at p < 0.05. A total of 312 fractures (91 pilon only, 193 calcaneal only, and 14 ankles with ipsilateral pilon and calcaneal fractures) were identified in 273 patients. Twenty-two pilon, 42 calcaneal, and nine combination fractures were associated with 99 occurrences of tendon entrapment or superior peroneal retinacular injury. Such findings were associated with multiple fractures (p = 0.002). Multifragmentary pilon fractures were associated with posterior tibial and flexor digitorum longus tendon entrapment (p < 0.0001 and p = 0.0003 for Ruedi/Allgower and AO-OTA, respectively), and multifragmentary Sanders type 3 or 4 calcaneal fractures were associated with superior peroneal retinacular injury (p = 0.0473) compared to simple fracture patterns. Thirty-nine percent of tendon entrapments or retinacular injuries were prospectively identified, 85 % by musculoskeletal radiologists (p < 0.0001). Approximately 25 % of calcaneal and pilon fractures were retrospectively identified to contain posteromedial tendon entrapment or superior peroneal retinacular injury. Radiologists should meticulously search for such injuries, particularly when analyzing multifragmentary and multiple fractures. (orig.)

    Background Few data on ankle fractures in China from large multicenter epidemiological and clinical studies are available.The aim of this research was to evaluate the epidemiological features and surgical outcomes of ankle fractures by reviewing 235 patients who underwent ankle fracture surgery at five hospitals in China.Methods This study included patients who underwent ankle fracture surgery at five Chinese hospitals from January 2000 to July 2009.Age,gender,mechanism of injury,Arbeitsgemeinschaft für Osteosynthesefragen (AO) fracture type,fracture pattern,length of hospital stay and treatment outcome were recorded.Statistical analyses were conducted using SPSS software.The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale,visual analogue scale (VAS),and arthritis scale were used to evaluate outcome.Results Of 235 patients with ankle fractures,105 were male with an average age of 37.8 years and 130 were female with an average age of 47.3 years.The average follow-up period was 55.7 months.There were significant differences in the ratios of patients in different age groups between males and females,and in mechanisms of injury among different age groups.There were also significant differences in the length of hospital stay among different fracture types and mechanisms of injury.In healed fractures,the average AOFAS ankle-hindfoot score was 95.5,with an excellence rate of 99.6%,the average VAS score was 0.17,and the average arthritis score was 0.18.Movement of the injured ankle was significantly different to that of the uninjured ankle.There were no significant differences between AO fracture types,fracture patterns or follow-up periods and AOFAS score,but there were some significant differences between these parameters and ankle joint movements,pain VAS score and arthritis score.Conclusions Ankle fractures occur most commonly in middle-aged and young males aged 20-39 years and in elderly females aged 50-69 years.The most common mechanisms of

    Mechanical ventilation (MV) is an essential part of modern intensive care medicine. MV is performed in patients with severe respiratory failure caused by insufficiency of the respiratory muscles and/or lung parenchymal disease when/after other treatments, i. e. oxygen, body position, secretion management, medication or non invasive ventilation have failed.In the majority of ICU patients weaning is routine and does not present any problems. Nevertheless 40-50 % of the time during mechanical ventilation is spent on weaning. About 20 % of patients need continued MV despite resolution of the conditions which originally precipitated the need for MV.There maybe a combination of reasons; chronic lung disease, comorbidities, age and conditions acquired in ICU (critical care neuromyopathy, psychological problems). According to an International Consensus Conference the criteria for "prolonged weaning" are fulfilled if patients fail at least three weaning attempts or require more than 7 days of weaning after the first spontaneous breathing trial. Prolonged weaning is a challenge. An inter- and multi-disciplinary approach is essential for weaning success. Complex, difficult to wean patients who fulfill the criteria for "prolonged weaning" can still be successfully weaned in specialised weaning units in about 50% of cases.In patients with unsuccessful weaning, invasive mechanical ventilation has to be arranged either at home or in a long term care facility.This S2-guideline was developed because of the growing number of patients requiring prolonged weaning.   6. Clinical effect of distal radius fracture treated with open reduction and internal plate fixation

    Background For some specific comminuted unstable intra-articular fracture,the plaster cast can not maintain the alignment of the articular surface effectively.The aim of this study was to evaluate the clinical effects of distal radius fracture treated with open reduction and internal plate fixation retrospectively.Methods From January 2002 to March 2010,539 cases of distal radius fracture were treated with open reduction and internal fixation,including 184 males and 355 females aging 21-72 years (mean 57 years).Fractures were caused by falling to the ground in 459 cases,by traffic accident in 62 cases and by athletic injuries in 18 cases.Of 539 cases,there were 523 cases of closed fracture and 16 cases of open fracture.According to Arbeitsgemeinschaft fur Osteosynthesefragen (AO) standards of classification,there were 14 cases of A2 type,22 of A3 type,18 of B1 type,24 of B2 type,62 of B3 type,91 of C1 type,162 of C2 type and 146 of C3 type.The time from injury to operation was 1-16 days (mean 5 days).All patitents received open reduction and internal plate screw fixation.Forty-seven patients with bone defect were given 6-15 g autologous ilium and 75 cases were given 5 ml calcium sulphate artificial aggregate after reduction.Results All incisions healed by first intention after operation.Patients were followed up for 15 to 32 months postoperatively (mean 22 months).The fractures healed within 10-18 weeks after operation (mean 12 weeks).During the last follow-up,the mean palmar tilt was (7.0±0.9)° and the mean ulnar variance was (21.0±4.2)°,showing significant difference when compared with preoperation ((-5.0±1.2)° and (8.0±3.8)°).The radial heights were not abbreviated.According to Gartland and Werley assessment system,the results were excellent in 314 cases,good in 163 cases,fair in 46 cases,and poor in 16 cases 12 weeks after operation,the excellent and good rate was 88.5%.Conclusions The clinical effect of distal radius fracture treated with open

    -Rekonstruktionen klassifiziert und die Ergebnisse mit konventionellen Roentgenaufnahmen (CR) und der Spiral-CT als etabliertem Standardverfahren verglichen. Die Diagnose wurde makroskopisch durch das Freipraeparieren der Kniegelenke gesichert. Ergebnisse: Mit dem Prototyp ISO-C-3D konnten 97% der Tibiakopffrakturen richtig erkannt und nach der AO-Einteilung (Arbeitsgemeinschaft fuer Osteosynthese) klassifiziert werden. Die Frakturdarstellung in Bezug auf Frakturart und -ausmass ist der CT gleichwertig. Die Erkennbarkeit und die korrekte Klassifizierung von Frakturen ist im ISO-C-3D im Vergleich zum CR signifikant besser. (orig.)

  8. Lateral migration with telescoping of a trochanteric fixation nail in the treatment of an intertrochanteric hip fracture

    Background The trochanteric fixation nail (TFN) can be used to treat stable and unstable fractures of intertrochanteric hip fractures.We study the common lateral migration that occurs with telescoping of intertrochanteric hip fractures treated with TFN and identify the predictors and relationships to clinical outcomes.Methods Patient demographic information,fracture type (Arbeitsgemeinschaft für Osteosynthesefragen (AO)/Orthopaedic Trauma Association (OTA) classification),radiographic data,and clinical data were collected.Lateral migration with telescoping was measured.Statistical analyses were performed to determine which variables predicted lateral migration with telescoping.Patient outcome scores were recorded using the Modified Harris Hip Score (MHHS),Hip Outcome Score-Activity of Daily Living (HOS-ADL),and Visual Analog Scale for pain.Results Two hundred and twenty-three patients (67 males,156 females) fitted the radiographic and follow-up (average 24.6 months) criteria.The average age was 77.2 years.The average lateral migration with telescoping was 4.8 mm.Twenty-one patients (9.4%) had excessive lateral migration with telescoping (≥10 mm).The quality of calcar reduction (P=0.01) and unstable fracture patterns (P=0.006) were significant predictive factors of lateral migration with telescoping.The mean outcome scores (MHHS and HOS-ADL) were 80.1 points and 78.7 points,respectively.All subjects had no significant relationship to lateral migration with telescoping (P >0.05).Of all the patients who developed lateral migration with telescoping,only one required removal of the blade for hip pain and all patients went on to uneventful union at an average time of 4.5 months.Conclusions Lateral migration with telescoping is a common mechanical complication of intertrochanteric hip fracture treated with the TFN procedure.It was predicted by the quality of calcar reduction and fracture type.However,this did not affect stable fixation and fracture healing

    Purpose: To evaluate whether a new software from the working group for interventional radiology (AGIR) is an appropriate tool for quality assurance in interventional radiology, and presentation of results acquired within the quality improvement process in 1999. Patients and methods: AGIR-defined parameters such as patient data, risk profile, given interventions as well as complications were registered by a recently developed software. Based on monthly data analyses, possible complications were identified and discussed in morbidity and mortality conferences. Results: 1014 interventions were performed in our institution in 1999. According to criteria established by AGIR, the complication rate was 2.7%. In addition and according to SCVIR criteria, complications were distinguished quantitatively in five classes and semiquantitatively in minor and major groups. The result was a minor complication rate of 1.8%, and a major rate of 0.9%. There were no cases of death associated with the intervention. Further strategies were developed in order to reduce the complication rate. Conclusion: Extensive quality assurance methods can be integrated in daily routine work. These methods lead to an intensive transparency of treatment results, and allow the implementation of continuous quality improvements. The development of the software is a first step in establishing a nation-wide quality assurance system. Nevertheless, modification and additional definition of the AGIR predefined parameters are required, for example, to avoid unnecessary procedures. (orig.)     Anwendung der Spiral-CT koennen entscheidende Zusatzinformationen zur Morphe der Fraktur, damit zur Einschaetzung des Frakturtyps und letztlich zur Indikationsstellung einer ggf. operativen Therapie gewonnen werden. Die MRT ist hervorragend geeignet, wertvolle weiterfuehrende Informationen in Bezug auf Verletzungen der diskoligamentaeren Anteile der Wirbelsaeule zu geben. Speziell fuer die Verletzungen der Brust- und Lendenwirbelsaeule entwickelten Magerl et al. eine umfassende Klassifikation, die von der Arbeitsgemeinschaft fuer Osteosynthesefragen (AO) uebernommen wurde. Grundlage ist ein 2-Saeulen-Modell der Wirbelsaeule. Als Basis der Einteilung dienen bildgebend erkennbare pathomorphologische Charakteristika der Frakturen. Das Verletzungsmuster wird besonders beruecksichtigt. Beim Wirbelsaeulentrauma wird zwischen stabilen und instabilen Frakturen unterschieden. Die Behandlung des Wirbelsaeulenverletzten richtet sich nach dem Schweregrad der Gesamtverletzung. Neben einer adaequaten Erstversorgung am Unfallort sollte umgehend ein Trauma-CT durchgefuehrt werden, um keine Verletzungen zu uebersehen und eine schnelle Therapieentscheidung herbeizufuehren.     The author investigated the effects of pressure in the gear casing on the load-independent meshing losses up to circumferential speeds of v{sub t} = 200 m/s. Apart from the reduction of losses, the limits of pressure drop are of interest. As the oil retains its characteristics even in partial vacuum, neither oil components nor additives must evaporate during operation. This means that also the effects of partial vacuum on the physical mechanisms was investigated as these may induce explosions of the oil spray inside the gear. Also considered are the characteristics of oil and air which are required for the design of the suction system. The investigations were carried out in close cooperation with Messrs. BHS-Cincinnati, Sonthofen, with financial aid from the Arbeitsgemeinschaft Industrieller Forschungsvereinigungen (AIF). [German] Turbogetriebe werden vorzugsweise zur Drehzahlwandlung zwischen Turbinen und Generatoren oder Verdichtern eingesetzt. Die groessten bekannten Ausfuehrungen solcher Getriebe liegen derzeit bei Leistungen bis P{sub A}=100 MW mit Antriebsdrehzahlen bis n{sub T}=7200 min{sup -1}. Mit der stetigen Weiterentwicklung der Antriebsmaschinen wachsen auch die Anforderungen an die Turbogetriebe. Derzeit befinden sich Anlagen mit P{sub A}=140 MW und Getriebeeingangsdrehzahlen bis n{sub 1}=10 000 min{sup -1} im Planungsstadium. Mit der Umfangsgeschwindigkeit steigen die Ventilations- und Quetschverluste ueberproportional an. Die daraus resultierende groessere Verlustwaerme fuehrt dazu, dass die Probleme bezueglich Kuehlung und Schmierung fuer den Getriebebauer zunehmend in den Vordergrund treten. Im Betrieb reicht der zur Schmierung benoetigte Oelvolumenstrom nicht zur Kuehlung der Getriebe aus, der groesste Teil des eingespritzten Oelvolumenstroms wird zur Kuehlung benoetigt.   12. Outcome of Radial Head Arthroplasty in Comminuted Radial Head Fractures: Short and Midterm Results

    Full Text Available Background Comminuted radial head fractures are often associated with secondary injuries and elbow instability. Objectives The aim of this retrospective study was to evaluate how well the modular metallic radial head implant EVOLVE® prosthesis restores functional range of motion (ROM and stability of the elbow in acute care. Patients and Methods Eighty-five patients with comminuted radial head fractures and associated injuries received treatment with an EVOLVE® prosthesis between May 2001 and November 2009. Seventy-five patients were available for follow-up. On average, patients were followed for 41.5 months (33.0: 4.0 - 93.0. Outcome assessment was done on the basis of pain, ROM, strength, radiographic findings, and functional rating scores such as Broberg and Morrey, the Mayo elbow performance index (MEPI, and disabilities of the arm, shoulder and hand (DASH. Our study is currently the largest analysis of clinical outcome of a modular radial head replacement in the literature. Results Overall, there were 2 (2.7% Mason II fractures, 21 (28% Mason III fractures, and 52 (69.3% Mason IV fractures. Arbeitsgemeinschaft fur osteosynthesefragen (AO classification was also determined. Of the 85 patients in our study, 75 were available for follow-up. Follow-up averaged 41.5 months (range, 4 - 93 months. Average scores for the cohort were as follows: Morrey, 85.7 (median 90.2; range 44.4 - 100; MEPI, 83.3 (85.0; 40.0 - 100; and DASH 26.1 points (22.5; 0.0 - 75.8. Mean flexion/extension in the affected joint was 125.7°/16.5°/0° in comparison to the noninjured side 138.5°/0°/1.2°. Mean pronation/supination was 70.5°/0°/67.1° in comparison to the noninjured side 83.6°/0°/84.3°. Handgrip strength of the injured compared to the non-injured arm was 78.8%. Bei der Abgrenzung zwischen ''Heilkunde'' und ''medizinischer Forschung'' kommt es bei radioonkologischen Studien immer wieder zu widerspruechlichen Einschaetzungen. Ziel ist es, eine Handreichung fuer Antragsteller, im Beantragungsprozess involvierte Personen und Institutionen sowie Ethikkommissionen zu liefern. Die Inhalte sind das Resultat eines von der Deutschen Gesellschaft fuer Radioonkologie (DEGRO) zusammen mit der Arbeitsgemeinschaft Radioonkologie (ARO) der Deutschen Krebsgesellschaft, der Deutschen Gesellschaft fuer Medizinische Physik (DGMP) und dem Deutschen Konsortium fuer Translationale Krebsforschung (DKTK) veranstalten Workshops zur klinischen Forschung in der Radioonkologie im Oktober 2013 sowie des nachfolgenden Abstimmungsprozesses mit dem BfS.     治疗药物监测( Therapeutic drug monitoring,TDM),如通过定量测定血清或血浆药物浓度指导用药剂量优化,已经成为对患者进行精神药物治疗的很有价值的工具。在患者用药依从性难以判断、药物耐受性不佳、治疗剂量下无效以及可能存在药代动力学药物-药物相互作用等情况下,测定药物浓度是很有用的。在精神科,有可能明显获益于TDM的主要患者群体包括儿童、孕妇、老年患者、智力障碍患者、涉及司法的患者、已知或怀疑携带药代动力学相关基因变异的患者,以及合并躯体疾病影响药代动力学的患者。然而,只有将TDM充分整合到临床治疗过程中去,才能发挥其优化药物治疗的潜在优势。为了促进TDM的合理应用,神经精神药理学与药物精神病学协会( Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie,AGNP)的TDM专家组在2004年发表了精神药物治疗药物监测指南。之后,随着知识不断更新,又有许多可能需要进行TDM的新药上市。因此,本次更新将神经精神药物的种类扩展到了128种,并将其TDM必要性划分为从“强烈推荐”到“可能有用”的四个等级。经过大量细致且全面的文献检索与分门别类的汇总整理,将基于循证医学理念的“治疗参考浓度范围”和“剂量相关参考浓度范围”呈现给大家。本共识指南引入了“实验室警戒浓度”的新概念,即实验室需要马上告知治疗医生的药物浓度上限。本共识指南还给出了诸如药物作为细胞色素P450酶的底物和抑制剂的性质,代谢物与母药浓度比值的常见范围,以及与结果解释相关的内容,还提供了何时将 TDM与遗传药理学检测相结合的建议。遵循本指南,有助于改善许多患者精神药物治疗的效果,特别是那些存在药代动力学异常的患者。TDM是一门交

    Neutron and x-ray scattering have emerged as powerful methods for the determination of structure and dynamics. Driven by emerging new, powerful neutron and synchrotron radiation sources, the continuous development of new instrumentation and novel scattering techniques gives rise to exciting possibilities. For example, in situ observations become possible via a high neutron or x-ray flux at the sample and, as a consequence, morphological transitions with small time constants can be detected. This special issue covers a broad range of different materials from soft to hard condensed matter. Hence, different material classes such as colloids, polymers, alloys, oxides and metals are addressed. The issue is dedicated to the 60th birthday of Professor Winfried Petry, scientific director of the Research Neutron Source Heinz Maier-Leibnitz (FRM-II), Germany, advisor at the physics department for the Bayerische Elite-Akademie, chair person of the Arbeitsgemeinschaft Metall- und Materialphysik of the German Physical Society (DPG) and a member of the professional council of the German Science Foundation (Deutsche Forschungsgemeinschaft, DFG). We would like to acknowledge and thank all contributors for their submissions, which made this special issue possible in the first place.     The present guideline (S2k) on allergen-specific immunotherapy (AIT) was established by the German, Austrian and Swiss professional associations for allergy in consensus with the scientific specialist societies and professional associations in the fields of otolaryngology, dermatology and venereology, pediatric and adolescent medicine, pneumology as well as a German patient organization (German Allergy and Asthma Association; Deutscher Allergie- und Asthmabund, DAAB) according to the criteria of the Association of the Scientific Medical Societies in Germany (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften, AWMF). AIT is a therapy with disease-modifying effects. By administering allergen extracts, specific blocking antibodies, toler-ance-inducing cells and mediators are activated. These prevent further exacerbation of the allergen-triggered immune response, block the specific immune response and attenuate the inflammatory response in tissue. Products for SCIT or SLIT cannot be compared at present due to their heterogeneous composition, nor can allergen concentrations given by different manufacturers be compared meaningfully due to the varying methods used to measure their active ingredients. Non-modified allergens are used for SCIT in the form of aqueous or physically adsorbed (depot) extracts, as well as chemically modified allergens (allergoids) as depot extracts. Allergen extracts for SLIT are used in the form of aqueous solutions or tablets. The clinical efficacy of AIT is measured using various scores as primary and secondary study endpoints. The EMA stipulates combined symptom and medication scores as primary endpoint. A harmonization of clinical endpoints, e. g., by using the combined symptom and medication scores (CSMS) recommended by the EAACI, is desirable in the future in order to permit the comparison of results from different studies.
