    In 1994, the rural districts of Darmstadt-Dieburg, Bergstrasse, Odenwald and Gross-Gerau in southern Hessen and the town of Darmstadt concluded a cooperation agreement permitting them to meet the future demands of the technical code on municipal waste and the `Kreislaufwirtschafts- und Abfallgesetz` (act concerning waste recycling and waste management). In order to formally prepare different forms of cooperation and to accompany corresponding preliminary investigations, the Suedhessische Arbeitsgemeinschaft Abfallwirtschaft (SAGA) was founded as a municipal task force. SAGA`s task is to debate common affairs in the waste management sector with a view to joint planning and joint facilities. In particular, the requirements of the technical code on municipal waste must be met. (orig.) [Deutsch] Im Jahre 1994 haben die suedhessischen Landkreise Darmstadt-Dieburg, Bergstrasse, Odenwald, Gross-Gerau sowie die Stadt Darmstadt beschlossen, bezueglich der kuenftigen Anforderung der TA Siedlungsabfall und des Kreislaufwirtschafts- und Abfallgesetzes zu kooperieren. Zur formellen Vorbereitung von verschiedenen dann auszufuehrenden Kooperationsformen und zur Begleitung entsprechender Voruntersuchungen wurde die Suedhessische Arbeitsgemeinschaft Abfallwirtschaft (SAGA) als Kommunale Arbeitsgemeinschaft gegruendet. Aufgabe der Kommunalen Arbeitsgemeinschaft ist es, gemeinsam beruehrende Angelegenheiten im Abfallbereich unter dem Gesichtspunkt gemeinsamer Planung und gemeinsamer Einrichtungen zu beraten. While the 'classic' parameters, i.e. nutrients, salts, heavy metals, summary organic substance parameters have reached have reached a more or less satisfactory and stable level, there are always enhanced concentrations or new compounds of anthropogenic zenobiotics. Jahresbericht vor, in dem ueber ihre Taetigkeiten, Ergebnisse aus dem Untersuchungsprogramm der Rhein-Wasserwerke and aktuelle Fragestellungen berichtet wird. In diesem Jahrsbericht wird wiederum schwerpunktmaessig auf die Beschaffenheit des Rheinwassers unter dem Aspekt der Trinkwassergewinnung eingegangen. Waehrend bei den klassischen Parametern (Naehrstoffe, Salze, Schwermetalle, summarische organische Messgroessen) inzwischen ein ueberwiegend guter chemischer Zustand erreicht ist, werden bei den anthropogenen xenobiotischen Einzelstoffen immer wieder erhoehte Konzentrationen oder 'neue' Verbindungen im Rhein entdeckt.     This is the 57th annual report of the Arbeitsgemeinschaft Rhein-Wasserwerke e.V. (ARW) on Rhine water quality from the view of drinking water supply. The positive trends continued in some fields, especially in the case of chloride concentrations and freights. On the other hand, there was a significant increase of some organic pollutants like DTPA, glyphosate and AMPA. The microbiological specifications of the new Drinking Water Ordinance and the concentrations of tensides and their metabolites are gone into. The ARW is worried about new or newly identified organic substances in Rhine water, especially as there is a lack of toxicological data on these substances and of data concerning their behaviour during freshwater preparation for drinking. [German] Die Arbeitsgemeinschaft Rhein-Wasserwerke e.V. (ARW) legt hiermit ihren nunmehr 57. Jahresbericht vor, in dem ueber ihre Aktivitaeten und wichtige Ergebnisse aus dem Untersuchungsprogramm der Rhein-Wasserwerke berichtet wird. Wie in den Vorjahren steht im Mittelpunkt der fachlichen Arbeiten der ARW die Bewertung der Rheinwasserbeschaffenheit aus Sicht der Trinkwasserversorgung. Im ersten Teil dieses Jahresberichtes sind die wichtigsten Ergebnisse zusammengefasst, die insgesamt erkennen lassen, dass sich die positiven Tendenzen bei vielen Wassergueteparametern auch im Berichtsjahr 2000 fortgesetzt haben. Dies gilt vor allem fuer die Chlorid-Konzentrationen und -Frachten im Rhein, die im Vergleich zum Vorjahr deutlich zurueckgegangen sind. Im Gegensatz dazu haben die Konzentrationen einiger organischer Einzelstoffe, wie z. B. DTPA sowie Glyphosat und AMPA, im Jahr 2000 z. T. deutlich zugenommen.     The Weser is Germany's only river whose catchment area is entirely in Germany. This opens up interesting options in terms of water quality investigations, water protection measures, and river management. The ARGE Weser working group, which comprises the German states of Bremen, Hessen, Niedersachsen, Nordrhein-Westfalen and Thueringen, has been carrying out long-term measurements for many years in selected sites on the Werra, Fulda, Weser and Aller rivers. This report presents data, as well as information on the concentrations and action of the analyzed substances. It also presents an evaluation and assessment of the selected substances. [German] Das Einzugsgebiet der Weser befindet sich vollstaendig innerhalb der Bundesrepublik Deutschland. Im Vergleich zu anderen bedeutenden deutschen Flusseinzugsgebieten sind mit dieser Besonderheit aussergewoehnliche Moeglichkeiten verbunden. So koennen Gewaesseruntersuchungen, Gewaesserschutz und Bewirtschaftungsmassnahmen effektiv und ohne Hemmnisse durchgefuehrt und umgesetzt werden. Die Arbeitsgemeinschaft zur Reinhaltung der Weser (ARGE Weser), der die Bundeslaender Bremen, Hessen, Niedersachsen, Nordrhein-Westfalen und Thueringen angehoeren, fuehrt seit vielen Jahren an ausgewaehlten Standorten der Werra, Fulda, Weser und Aller Untersuchungen zur Wasserbeschaffenheit durch.     The Arbeitsgemeinschaft Fernwaerme (AGFW) has developed a branch-specific maintenance-planning and control system for its own association. In co-operation with its member companies a functional requirement catalogue for an IPS-system and the corresponding data model have been elaborated in the frame of a joint venture project. An investigation and evaluation of the systems offered at the market led to the decision to develop an own system for the association. The Beta-version will probably be available by June 1995.     Work on the high-temperature gas turbine started in October 1986 in the context of AG Turbo. The 5th status seminar took a look back, with 19 papers on the fluid mechanics of turbomachinery, analysis and mathematical description of flow phenomena. (orig./AKF) [Deutsch] Vor 10 Jahren, im Oktober 1986, wurden die ersten Entwicklungsarbeiten an der Hochtemperatur Gasturbine im Rahmen der AG Turbo begonnen. Dieses Ergebnis veranlasst uns heute, am Beginn des 5. Statusseminars, zu einem Rueckblick mit 19 Beitraegen, die im vorliegenden Bericht enthalten sind. Turbotech befasst sich mit der Verbesserung der fluidmechanischen Energieumsetzung in Turbomaschinen. Hier wurden schwerpunktmaessig Fragen behandelt, die sich mit der Analyse und mathematischen Beschreibung der realen Stroemung hochbelasteter Turbinen und Verdichter befassen. Die Beherrschung der hohen Temperaturen, insbesondere vor der Turbine, steht im Mittelpunkt des Interesses von Turbotherm. Fuer die Turbine konnten neue Herstellungstechnologien und erstmals keramische Werkstoffe auf ihre Eignung zum Einsatz in dieser untersucht werden. Aufgabe von Turboflam ist die Entwicklung moderner hochbelasteter Brennkammern mit einer stabilen, schadstoffarmen Verbrennung fuer fluessige und gasfoermige Brennstoffe. Zum Erreichen des Entwicklungszieles wurden die beiden Arbeitsschwerpunkte Brennkammermodellierung und Entwicklung neuer Brennkammerkonzepte gebildet. (orig./AKF)

    The purpose of the meeting was to provide a forum for exchange of information on safety and licensing aspects for gas-cooled reactors in order to provide comprehensive review of the present status and of directions for future applications and development. Contributions were made concerning the operating experience of the Fort St. Vrain (FSV) HTGR Power Plant in the United States of America, the experimental power station Arbeitsgemeinschaft Versuchsreaktor (AVR) in the Federal Republic of Germany, and the CO/sub 2/-cooled reactors in the United Kingdom such as Hunterson B and Hinkley Point B. The experience gained at each of these reactors has proved the high safety potential of Gas-cooled Reactor Power Plants.

    An increasing variety of computed tomography scanners with different imaging geometry and diverse protocol recommendations of the vendors for chest imaging have raised the need for a consensus on basic imaging protocols. In addition to this, radiation exposure has become a relevant issue in computed tomography. With the recommendations of this article it was intended to provide a schematic set of protocols and recommendations for computed tomography of the lung, i.e. to facilitate follow-up reads of studies that were acquired with different scanners and at different sites. Four different types of protocols are suggested for different groups of indications. The basic protocol is a contrast-enhanced helical study with sufficient spatial resolution for tumour staging and workup of unclear situations. The second protocol is a refinement of this protocol with increased spatial resolution dedicated to vessel imaging and computeraided (CAD) applications. Depending on the z-coverage, this infers a higher radiation exposure and more data for post-processing and storage. A low dose protocol is recommended for short term follow up of known disease and imaging focused on the lung parenchyma only. For diffuse lung disease, an additional HRCT-mode inincremental technique can be applied. Adapted protocol recommendations are given for three scanner groups (single row detector and twin, 4-16 row detector and 32 and more row detectors). Nevertheless, individual adjustments for all patients are essential and some of the parameters will need to be adjusted for specific scanners and may thus differ from these general recommendations. Finally, we make suggestions for documentation, reporting and archiving (preferably digital). (orig.)

  15. Thirteen years of independent radioactivity monitoring: Arbeitsgemeinschaft Umgebungsueberwachung von Atomanlagen (AUA)

    Some 13 years ago local citizens' initiatives at several sites of nuclear power plants have consolidated their formal structure as registered societies. One of the goals of these societies is to critically watch and monitor the routine operation and all related activities of 'their' local nuclear power plant in order to maintain continous public interest, discussion, and concern. As one important means to achieve this societies try to measure radioactive emissions from the plant and publish the results regularly. For this, professional equipment for continously monitoring of gamma-submersion in air was installed in distances of 1 to 5 km around each observed power plant. These devices also collect important weather data, which are needed for valid and reliable interpretation of radioactive immission data. Physicists and engineers developed concepts, selected the equipment, and perform the technical and scientific evaluation, while installing, serving and maintaining the apparatus, interpretation of data, and publishing of the results is performed by the local societies. Regular meetings of all groups and individuals collaborating in this project (named 'AUA') help to share their acquired know-how and to boost further qualification. (orig.)

  18. MHTGR fuel design and quality

    The modular high-temperature gas-cooled reactor (MHTGR) has a top-level goal of safe, economical energy production. Occupational and site boundary irradiation exposure during normal and accident conditions is maintained well below established limits by multiple passive barriers to fission product release. The primary containment for fission products is the coated fuel particle, which consists of a microsphere of ceramic fuel encapsulated by an impervious, refractory coating. More than 1010 of these miniature pressure vessels are used in each MHTGR core. The design of the fissile fuel particle is shown. The design of coated particles and the quality achieved have improved significantly over the last two decades in both the United States and the FGR. In the United States the Fort St. Vrain HTGR fuel production set an early standard for quality. This standard was improved by a factor of 20x reduction in defects through the U.S. Department of Energy (DOE) development program for the earlier 2240-MW(thermal) HTGR and the current MHTGR. In the FRG the quality has improved from the Triso fuel made for the Arbeitsgemeinschaft Versuchsreaktor to current fuel for the gas-cooled reactor designs being developed there. The evolution in fuel quality coupled with the ongoing DOE-sponsored process development and fuel proof test program assures that the MHTGR design goal will be met

    Reviews were conducted at Kernforschungsanlage (KFA) Juelich of the US and Federal Republic of Germany (FRG) high-temperature gas-cooled reactor (HTGR) programs under the US/FRG Umbrella Agreement, with emphasis on those technology development areas where cooperation is ongoing and planned. Specific subprogram areas are safety; materials; fuels, fission products, and graphite; and Arbeitsgemeinschaft Versuchs-Reaktor (AVR). The purpose was to assess the status of the cooperation, reach agreement on any changes needed, and identify new areas of cooperation. Overall, the agreement has been both effective and beneficial. Ongoing activities complement and support US technology development plans. Discussions were held in the United Kingdom (UK) at the Risley Nuclear Power Development Laboratory regarding a potential graphite technology exchange program between the US Department of Energy and the UK Atomic Energy Authority. A draft agreement was reviewed and appeared to be satisfactory to both parties and ready for signature. A summary of potential areas of activity in the exchange had been prepared by US representatives and was discussed and found to be acceptable to UK representatives.

  20. Preliminary operating experiences with the AVR at an average hot-gas temperature of 9500C

    The two-loop system with a high temperature reactor, which is operated by the Arbeitsgemeinschaft Versuchsreaktor (AVR) GmbH and which was built by the BBC/Krupp consortium (today HRB), has been in operation for more than seven years. In that time more than 635 x 106kWhr have been produced and more that 6.5 x 105 spherical fuel elements have been circulated under operation. The fully integrated design, and above all the ceramic gas duct, permit very high gas temperatures although no high alloyed, heat resistant steels were used in the reactor. In February 1974 the average hot-gas temperature at the outlet of the core could thus be increased from its original design value of 8500C to 9500C. Peak temperatures of above 500C are thereby confined to a small region between the middle of the core and the beginning of the steam generator. Carbon protects the steel structures against high temperatures. Unplanned interruptions and reductions of operation due to the increase of the hot-gas temperature have not occurred so far. Some thermocouples in the hot-gas region failed. All other components functioned satisfactorily and, one year after the increase in the hot-gas temperature, there are no misgivings as to their future functioning. These satisfactory but short operating experiences at 9500C will have to be supplemented in the next few years by experiences over a longer period. (Auth.)

    In 1979-1980, breams caught between Schnackenburg and Glueckstadt were analyzed for heavy metals and chlorinated hydrocarbons. After the German unification, the analyses were extended to further sections of the river. The report presents the results. [German] In den Jahren 1979-1980 wurden Brassen zwischen Schnackenburg und Glueckstadt fuer eine gezielte Analyse auf verschiedene Schwermetalle und Chlorkohlenwasserstoffe gefangen. Vorrangiges Ziel der damaligen ARGE-ELBE-Untersuchung war die Eignung des Weissfisches als Bioindikator zur Darstellung der Schadstoffbelastung in der Elbe. Nach der Wiedervereinigung wurde ein weiteres Sondermessprogramm mit dem Brassen durchgefuehrt. Der Brassen wurde an 10 Fangplaetzen im bundesdeutschen Elbelaengsschnitt 1994 befischt: An zwei Stationen in der Oberen Elbe, an drei Fangstellen in der Mittelelbe und an fuenf Fangorten in der Tideelbe. Die Befunde wurden hinsichtlich regionaler Belastung und Vermarktungsfaehigkeit dargestellt. In dem vorliegenden Bericht werden ausnahmslos die Schadstoffgehalte der Brassenfaenge der Elbe des Jahres 1994 ausgewertet. Zur Festlegung der Hintergrundbelastung (Referenzstandard) des Bewertungssystems wurden Brassen aus voraussichtlich gering belasteten Gewaessern beruecksichtigt. Ziel der Datenauswertung ist eine Klassifizierung, welche die regionalspezifische Kontamination im Muskelfleisch der Elbe-Brassen widerspiegelt. (orig.)

    The present report contains results on the occurrence of polar and nonpolar substances in the Elbe and selected tributaries which were analysed in the period from 1994 to 1999. It shows that industrial and municipal purification plants still have an insufficient cleaning capacity for certain substances. The process by which ubiquitous pollutants can enter the Elbe via a small, widely ramified fluvial system is exemplified for the family of chloroalkyl phosphates. These mainly occur in flame retardants and plasticisers in polyurethanes. The paper also presents results from a selection of 28 substances listed in Directive 76/464/ECC. They provide a decision aid for the planning of future measuring concepts based on the EU Water Framework Directive. In the ARGE ELBE programme organic pollutants are recorded and published on a routine basis. These are mostly nonpolar substances such as the well-known polychlorinated biphenyls or slow-evaporating insecticidal chlorinated hydrocarbons such as DTT. Recent analysis results show that this group of substances is relevant now as before and that it will remain so for the years to come. Special studies involving aqueous phase measurements were performed on a different group of substances, namely polar substances, which are generally readily soluble. The substances on which this report is chiefly focused originate from different sources: industrial and municipal effluents, diffuse inputs from agriculture and atmospheric depositions. In many cases they are difficultly degradable and could attain significance where river water in the form of river bank filtrate is used for drinking water production. However, due to complex and effective purification technologies they rarely enter drinking water. [German] Der vorliegende Bericht enthaelt Ergebnisse zum Vorkommen polarer und unpolarer Stoffe in der Elbe und in ausgewaehlten Nebenfluessen, die in dem Zeitraum von 1994 bis 1999 analysiert worden sind. Er zeigt, dass fuer einzelne Stoffe noch Defizite im Hinblick auf die Reinigungsleistung industrieller und kommunaler Klaeranlagen existieren. Wie ubiquitaer vorhandene Schadstoffe ueber ein kleines und weitverzweigtes Fliesssystem in die Elbe gelangen koennen, wird beispielhaft an der Stoffklasse der Chloralkylphosphate aufgezeigt. Diese finden hauptsaechlich Verwendung als Flammschutzmittel und Weichmacher in Polyurethanen. Ergebnisse einer Auswahl von 28 Stoffen aus der Richtlinie 76/464/EWG werden ebenfalls vorgestellt. Sie sind eine Entscheidungshilfe fuer die Planung zukuenftiger Messkonzepte auf der Grundlage der EU-Wasserrahmenrichtlinie. Mit dem Messprogramm der ARGE ELBE werden organische Schadstoffe routinemaessig erfasst und veroeffentlicht. Dabei handelt es sich ueberwiegend um unpolare Stoffe wie die hinlaenglich bekannten polychlorierten Biphenyle oder die schwerfluechtigen chlorierten Kohlenwasserstoffe mit insektizider Wirkung (z.B. DDT). Neuere Analysenergebnisse zeigen, dass diese Stoffgruppe nach wie vor relevant ist und auch in den kommenden Jahren nicht an Bedeutung verlieren wird. Eine andere Stoffgruppe, die polaren und im allgemeinen gut wasserloeslichen Substanzen, werden im Rahmen von Sonderuntersuchungen in der waessrigen Phase bestimmt. Die in diesem Bericht hauptsaechlich behandelten Stoffe stammen aus unterschiedlichen Quellen: industrielle und kommunale Abwaesser, diffuse Eintraege aus der Agrarwirtschaft und aus atmosphaerischen Depositionen. In vielen Faellen sind sie biologisch schwer abbaubar und koennen Bedeutung erlangen, wenn Flusswasser als Uferfiltrat fuer die Trinkwasserherstellung genutzt wird. Wegen der aufwendigen und wirkungsvollen Aufbereitungstechnologie gelangen sie allerdings nur selten bis ins Trinkwasser. (orig.)

    This special report focuses on measurements of elements whose scope goes far beyond the routine monitoring for metals. The present studies are intended to extend the existing knowledge on the pollution situation of the Elbe and certain of its tributaries as well as provide a basis for formulating future guidelines for action. The first step towards this goal is sampling by means of an helicopter. The sampling is followed by a very involved and costly analysis programme, accompanied by extensive quality assurance measures. This is the only way of reliably analysing a large number of elements occurring in extremely low concentrations. Once the data have been recorded the question arises how they are to be evaluated. What schemes are suitable for categorising a large number of elements measured in either the suspended matter or the water compartment? What elements are to be regarded as the result of anthropogenic influences and what are their respective background concentrations in the fluvial system under study? These questions are approached by means of statistical methods which permit identifying elemental patterns and pollution sources as well as optimising the selection of measuring points for future studies. [German] In diesem Sonderbericht steht die Bestimmung von Elementen, die in ihrem Umfang ueber die routinemaessig erfassten Metalle deutlich hinausgeht, im Vordergrund. Die vorliegenden Untersuchungen sollen das bereits vorhandene Wissen zur Beschreibung der Immissionssituation der Elbe und von bestimmten Nebenfluessen erweitern, verbunden mit der Moeglichkeit, zukuenftige Handlungsmaximen zu erarbeiten. Zur Erreichung dieses Ziels steht zu Beginn die Probenahme mit einem Helicopter. Im Anschluss an die Probenahme folgt eine sehr aufwendige und teure Analytik, die begleitet wird von umfassenden Massnahmen zur Qualitaetssicherung. Nur so ist es moeglich, eine grosse Vielzahl von Elementen in extrem niedrigen Konzentrationen sicher analysieren zu koennen. Sind die Daten erhoben, schliesst sich die Frage nach ihrer Bewertung an. Welche Schemata sind tauglich, um eine Vielzahl von Elementen, die in den unterschiedlichen Kompartimenten Schwebstoff und Wasser bestimmt werden, einstufen zu koennen? Welche Elemente sind als anthropogen beeinflusst zu bezeichnen, welche Konzentrationen stellen den background in dem betrachteten Flusssystem vor? Zur Beantwortung dieser Fragen wird sich der Statistik bedient, denn mit ihrer Hilfe koennen Elementmuster identifiziert, Belastungsquellen aufgezeigt und darueber hinaus die Auswahl von Messstellen fuer kuenftige Untersuchungen optimiert werden.     Endocrineously active substances are discharged into surface bodies of water with communal and industrial effluents. Especially estrogenic substances like Bisphenol A, alkyl phenols and alkyl phenol ethoxylates in the Elbe and its tributaries have been neglected so far. In the context of an interdisciplinary project, chemists, biologists and mathematicians investigated the concentrations and fate of these xenoestrogens, relative potential effects of unknown water constituents and quantitative statistical mechanisms based on the available analytical findings. The contribution intends to contribute information on the pollutant situation of selected endocrineously active substances in the Elbe and its tributaries. [German] Ein relevanter Eintragspfad fuer endokrin wirksame Stoffe in die Oberflaechengewaesser sind die Abwaesser kommunaler und industrieller Klaeranlagen. Insbesondere das Vorkommen und der Verbleib von oestrogen wirkenden Stoffen wie Bisphenol A, den Alkylphenolen und Alkylphenolethoxylaten in der Elbe und in Elbenebenfluessen wurde bisher zu wenig Aufmerksamkeit geschenkt. In einem interdisziplinaeren Projekt, an dem sich Chemiker, Biologen und Mathematiker beteiligten, wird versucht, eine erste Antwort zu geben ueber das Vorkommen und den Verbleib dieser Xenooestrogene, ueber relative Wirkpotenzen von unbekannten Wasserinhaltsstoffen durch die Anwendung von In-vitro-Tests und ueber quantitative statistische Zusammenhaenge, die auf vorhandenen Analysenergebnissen basieren. Dieser Bericht soll einen Beitrag leisten zur Immissionssituation von ausgewaehlten endokrin wirksamen Stoffen in der Elbe und in Elbenebenfluessen. (orig.)

  6. Nuclear energy research funding 1956-2002: Startup financing or subsidy?

    Different figures, some of them extremely large, some even interpreted as subsidies, have been publicized about public funding of the development of nuclear power for electricity generation. An attempt is made in this article to quantify the research funds directly attributable to the present use of nuclear power, put the research funds spent into a relationship to the electricity generated from nuclear power, and put a number on the economic benefit of the money spent. In the past, R and D work on the utilization of nuclear power was financed by various federal ministries. As in other technology-related areas, funds were made available for specific projects, on the one hand, and through financing the national research centers, on the other hand. Relatively small amounts were spent on the development of the light water reactor (LWR) line, most of them in the 1950s and 1960s. Considerably greater expenditures were incurred by the federal government for reactor safety. R and D of the fast breeder (LMFBR) was conducted chiefly between 1960 and 1993. Research into the high temperature (HTR) reactor line focused mainly on the plant built by the Arbeitsgemeinschaft Versuchsreaktor (AVR) association, and the thorium high temperature reactor (THTR-300) at a later date. Nuclear fuel cycle activities were started early in the 1950s. The decommissioning and dismantling of nuclear plants plays but a minor role in public funding. Nuclear fusion, on the other hand, is a major research area. The nuclear power plants now generating electricity from nuclear power have never received any subsidies, and their economic benefit is beyond any doubt. The public funds spent on R and D were intended to be startup funds. They convincingly served their purpose in those cases where the technologies supported in this way have been able to take off.   7. Radiochemical characterisation of graphite from Juelich experimental reactor (AVR)

    Graphite built-in nuclear reactors may receive a high neutron dose for a long period. Depending on its chemical composition a lot of activation products are produced. In addition, there is more or less fission product contamination depending on the location. The migration of fission products may be supported by high temperatures which occur in high temperature reactors. At the Juelich 15 MWe High Temperature Gas-cooled experimental Reactor AVR (Arbeitsgemeinschaft Versuchsreaktor) two different types of nuclear graphite had been in use. High-purity graphite was used as basic material for core structures of the AVR. Insulation layers from carbon bricks (graphite with larger amounts of impurities) surrounding the graphite reflector were used to protect the metallic structures from high temperatures. For many reasons it is important to know the amount of contamination of graphite and carbon bricks with activation products and fission products. The head end of nuclear graphite analytics must be the incineration. Volatile activities (14C, 3H, 36Cl ...) must be caught for determination. In case of handling dustlike samples the incineration furnace must be small enough to be operated in a glove box. The resulting ashes can be used for determining all non volatile nuclides with different radiochemical methods. In early 1999 some graphite and carbon brick samples from AVR-reactor had been taken by drilling. The samples had been analysed in our laboratories at Juelich research centre. For incineration we used a vertical quartz-tube which dips at the bottom into a small electric furnace. Tritium, 14C and 36Cl are caught in washing bottles. After further preparation, they are determined by LSC. After dissolving the ashes, the elements are separated by ion exchange, extraction methods and HPLC. The radionuclides are then determined by a-spectrometry, LSC, low level g-spectrometry and x-ray spectrometry. (author)

    The report provides the necessary background information required for understanding the measuring programme and its data and provides a parameter-specific discussion of the water quality on the basis of selected parameters. It addresses universities and public administrations but also interested laymen. The report shows inhowfar the goals defined in the 1989 programme for improving the water quality of the Weser river area have been reached: Reduction of water pollution of the Weser from hazardous substances; Reduction of nutrient pollution; Reduction of chloride pollution. The Weser water today is in quality range II of the biological water classification. This is a positive result. The chloride freight was reduced as well as pollution reduction measures were taken in the potassium mines of Thuringia. Further efforts are still required, however; they are defined in the new programme 'Aktionsprogramm Flussgebiet Weser 2000-2010' which already incorporates the specifications of the soon-to-be-issued EC water quality regulation. [German] Im Weserguetebericht werden die fuer das Verstaendnis notwendigen Hintergrundinformationen zu den Messprogrammen und den darin enthaltenen Kenngroessen ergeben und anhand von ausgewaehlten Darstellungen die Wasserqualitaet parameterspezifisch diskutiert. Die Art der Darstellung ist damit nicht nur auf den wissenschaftlich-technischen Bedarf von Fachdienststellen und Hochschulen ausgerichtet, sondern auch fuer den interessierten Leser nachvollziehbar. Darueber hinaus gibt der Guetebericht auch Auskunft, inwieweit die 1989 im Aktionsprogramm Weser formulierten Ziele zur Verbesserung der oekologischen Verhaeltnisse in und an der Weser erreicht werden: - Reduzierung der Belastung der Weser durch gefaehrliche Stoffe - Verringerung der Naehrstoffbelastung - Reduzierung der Chloridbelastung. Die Weser weist heute ueberwiegend die biologische Gewaesserguete II auf. Dies ist das positive Ergebnis des 1989 beschlossenen Aktionsprogrammes. Deutliche Erfolge wurden auch im Hinblick auf die Reduzierung der Chloridbelastung erzielt. So konnten gegen Ende 1998 die Massnahmen des Abkommens - zur Senkung der Salzbelastung - aus den thueringischen Kaliwerken erfolgreich abgeschlossen und 1999 in Betrieb genommen werden. Gleichwohl sind weitere Anstrengungen noetig, dies zeigt auch das neue Aktionsprogramm Flussgebiet Weser 2000-2010, das bereits die Vorstellungen und Anforderungen der kommenden EU-Wasserrahmenrichtlinie aufgreift.     Full Text Available [english] The Annual Meeting 2013 of the German MLA (AGMB e.V. took place from September 16 to 18 at the Charité – Universitätsmedizin Berlin. The conference was hosted by the Medical Library of the Charité.The main topics dealt with included evidence-based medicine and the role of librarians in the transfer of knowledge from research into practice, library services to support researchers in the publishing process, open access publishing, the role of libraries in research data management, pioneering models for the supply of digital natives with e-learning materials, as well as the requirements and tools of quality management in academic libraries. Work groups, meeting points and short presentations provided the participants with additional opportunities to gather information on current topics and to exchange their experiences. The programme was completed by a panel discussion on e-books, a workshop on systematic literature searching, an exhibition featuring the participating companies’ products and services, product reviews, a poster session and the prize giving event of the 2013 AGMB competition “Leuchttürme in Medizinbibliotheken”.

  11. Fuel-Cycle and Nuclear Material Disposition Issues Associated with High-Temperature Gas Reactors

    The objective of this paper is to facilitate a better understanding of the fuel-cycle and nuclear material disposition issues associated with high-temperature gas reactors (HTGRs). This paper reviews the nuclear fuel cycles supporting early and present day gas reactors, and identifies challenges for the advanced fuel cycles and waste management systems supporting the next generation of HTGRs, including the Very High Temperature Reactor, which is under development in the Generation IV Program. The earliest gas-cooled reactors were the carbon dioxide (CO2)-cooled reactors. Historical experience is available from over 1,000 reactor-years of operation from 52 electricity-generating, CO2-cooled reactor plants that were placed in operation worldwide. Following the CO2 reactor development, seven HTGR plants were built and operated. The HTGR came about from the combination of helium coolant and graphite moderator. Helium was used instead of air or CO2 as the coolant. The helium gas has a significant technical base due to the experience gained in the United States from the 40-MWe Peach Bottom and 330-MWe Fort St. Vrain reactors designed by General Atomics. Germany also built and operated the 15-MWe Arbeitsgemeinschaft Versuchsreaktor (AVR) and the 300-MWe Thorium High-Temperature Reactor (THTR) power plants. The AVR, THTR, Peach Bottom and Fort St. Vrain all used fuel containing thorium in various forms (i.e., carbides, oxides, thorium particles) and mixtures with highly enriched uranium. The operational experience gained from these early gas reactors can be applied to the next generation of nuclear power systems. HTGR systems are being developed in South Africa, China, Japan, the United States, and Russia.   12. Interstorage of AVR-Fuels in the Research-Center

    Between 26.08.1966 and 31.12.1988 the experimental nuclear power plant AVR was operated in the area of the Juelich research-center by the Arbeitsgemeinschaft Versuchs-Reaktor mbH, the AVR company. This plant was a Helium cooled high-temperature-reactor with an electric gross-power of 15 MW. This type of power plant was the first one being developed exclusively in Germany. The high-temperature-reactor AVR was one after the principle of the ball-pile-reactor developed by Professor Schulten. The core consists of spherical, graphite fuels with 60 mm diameter, that contain the fissile-material and breed-material in form of coated particles. The fuel is enclosed by a cylindrical graphite-construction which serves as the neutron-reflector. The coating of the fuel-particles consist of pyro-carbon and silicon-carbide and is used for the retention of the fission-products. The reactor has continuously been refueled by feeding the fuel balls into the core at the top and discharging them at the bottom during full operation. After the shut down the reactor now is on the way to safe closure while plans for dismantling have been started. The Juelich research-center was engaged with the storage of the spent fuels as part of the fuel management. The storage of the fuel in CASTOR(regsign) THTR/AVR casks is preceded by different actions, like the removal of the fuel from the reactor core, the interim storage of the fuel in AVR-cans in the buffer-storage, decanting of the fuel balls from AVR-cans in the dry-storage-cans (TLK), the interim storage of the TLK, welding of the TLK which contain wet fuel and the loading of each CASTOR(regsign) THTR/AVR cask with two TLKs, are necessary.     Full Text Available Abstract Background The activity of the epidermal growth factor receptor (EGFR-directed monoclonal antibody cetuximab combined with oxaliplatin/leucovorin/5-fluorouracil (FUFOX was assessed in first-line metastatic gastric and oesophago-gastric junction (OGJ cancer in a prospective phase II study showing a promising objective tumour response rate of 65% and a low mutation frequency of KRAS (3%. The aim of the correlative tumour tissue studies was to investigate the relationship between EGFR gene copy numbers, activation of the EGFR pathway, expression and mutation of E-cadherin, V600E BRAF mutation and clinical outcome of patients with gastric and OGJ cancer treated with cetuximab combined with FUFOX. Methods Patients included in this correlative study (n = 39 were a subset of patients from the clinical phase II study. The association between EGFR gene copy number, activation of the EGFR pathway, abundance and mutation of E-cadherin which plays an important role in these disorders, BRAF mutation and clinical outcome of patients was studied. EGFR gene copy number was assessed by FISH. Expression of the phosphorylated forms of EGFR and its downstream effectors Akt and MAPK, in addition to E-cadherin was analysed by immunohistochemistry. The frequency of mutant V600E BRAF was evaluated by allele-specific PCR and the mutation profile of the E-cadherin gene CDH1 was examined by DHPLC followed by direct sequence analysis. Correlations with overall survival (OS, time to progression (TTP and overall response rate (ORR were assessed. Results Our study showed a significant association between increased EGFR gene copy number (≥ 4.0 and OS in gastric and OGJ cancer, indicating the possibility that patients may be selected for treatment on a genetic basis. Furthermore, a significant correlation was shown between activated EGFR and shorter TTP and ORR, but not between activated EGFR and OS. No V600E BRAF mutations were identified. On the other hand, an interesting trend between high E-cadherin expression levels and better OS was observed and two CDH1 exon 9 missense mutations (A408V and D402H were detected. Conclusion Our finding that increased EGFR gene copy numbers, activated EGFR and the E-cadherin status are potentially interesting biomarkers needs to be confirmed in larger randomized clinical trials.   17. GRAFEC: A New Spanish Program to Investigate Waste Management Options for Radioactive Graphite - 12399

    Spain has to manage about 3700 tons of irradiated graphite from the reactor Vandellos I as radioactive waste. 2700 tons are the stack of the reactor and are still in the reactor core waiting for retrieval. The rest of the quantities, 1000 tons, are the graphite sleeves which have been already retrieved from the reactor. During operation the graphite sleeves were stored in a silo and during the dismantling stage a retrieval process was carried out separating the wires from the graphite, which were crushed and introduced into 220 cubic containers of 6 m3 each and placed in interim storage. The graphite is an intermediate level radioactive waste but it contains long lived radionuclides like 14C which disqualifies disposal at the low level waste repository of El Cabril. Therefore, a new project has been started in order to investigate two new options for the management of this waste type. The first one is based on a selective decontamination of 14C by thermal methods. This method is based on results obtained at the Research Centre Juelich (FZJ) in the Frame of the EC programs 'Raphael' and 'Carbowaste'. The process developed at FZJ is based on a preferential oxidation of 14C in comparison to the bulk 12C. Explanations for this effect are the inhomogeneous distribution and a weaker bounding of 14C which is not incorporated in the graphite lattice. However these investigations have only been performed with graphite from the high temperature reactor Arbeitsgemeinschaft Versuchsreaktor Juelich AVR which has been operated in a non-oxidising condition or research reactor graphite operated at room temperature. The reactor Vandellos I has been operated with CO2 as coolant and significant amounts of graphite have been already oxidised. The aim of the project is to validate whether a 14C decontamination can also been achieved with graphite from Vandellos I.     Presently axial compressors have to run with considerable distance to operation points of high load although usually the best efficiencies can be obtained there. In those areas there is high danger of surge which brings high damaging potential to the compressor. The prediction time to surge presently is very short (<<1s) and depends on the load and rotor rotation of the compressor. The goal of this project is the production of a data basis to investigate stall inception (which is the precursor of surge) and to develop and test better precursory warning algorithms. An extensive set of data as well in stable as in unstable operating points in different load areas was developed which now is ready for use in research, verification and for comparison of forewarning systems. [German] Zur Zeit ist es notwendig, Axialverdichter mit deutlichem Abstand von den hoch belasteten Betriebspunkten zu betreiben, obwohl haeufig dort die besten Wirkungsgrade zu erzielen waeren. In diesen Bereichen kann es leicht zu sog. Pumpen kommen, wodurch der Verdichter einem extremen Schaedigungspotential ausgesetzt ist. Die Vorwarnzeiten fuer das Pumpen sind bisher noch sehr kurz. Sie liegen in einem Bereich von deutlich weniger als einer Sekunde (abhaengig von der Belastung und der Drehzahl des Verdichters). Ziel dieser Untersuchungen ist es, eine Datenbasis zu liefern, anhand derer die Entstehungsmechanismen untersucht und moeglichst fruehzeitige Vorwarnalgorithmen entwickelt bzw. erprobt werden koennen. Es wurde eine umfangreiche Datenbasis sowohl an verschiedenen stabilen als auch instabilen Betriebspunkten in unterschiedlichen Lastbereichen erstellt, die nun zur Entwicklung, zur Verifizierung und zum Vergleich von Fruehwarnsystemen zur Verfuegung steht.   5. Advanced development of atomic energy in Germany and actual radioecological problems

    Full text: From 1956 a number of nuclear research centres were set up in West Germany, and most of these as well as university institutes were equipped with research rectors. Most of these reactors are now shut down and the centres have changed their roles. At the German Research Centre Juelich, an HTR, called the 'Arbeitsgemeinschaft Versuchsreaktor AVR', was opened in 1967. In this reactor the fuel was embedded in graphite balls. It was a thorium/uranium reactor; neutrons from chain reaction were used to breed fissionable uranium-233 from the non-fissionable thorium-232. Later models dropped the thorium cycles as there were too many problems with the reprocessing of thorium fuel. In 1985 the Hamm-Uentrop Thorium High Temperature Reactor (THTR) was opened. In May 1986 (a week after the Chernobyl accident), radioactive gas escaped from the cooling system, after graphite-fuel balls stuck in the fuel inlet. Other problems occurred when fuel balls were damaged and with sticking control rods. The Fast Breeder nuclear reactor SNR-300 was built near the town of Kalkar, Germany (located in the federal state of North-Rhine-Westphalia). It was completed but never taken online. It is known as one of the biggest government spending boondoggles in Germany. In 1963 the federal government issued a recommendation to use rock salt formations for radioactive waste disposal. In 1973 planning for a national repository started, and in 1976 the Atomic Energy Act was amended to make such disposal a responsibility of the federal government. From 1994 to 1998 reprocessing and direct disposal have been equally acceptable to the federal government, though previously utilities had been obliged to reprocess spent fuel. The policy of the coalition government from 1998 is for direct geological disposal of spent fuel and no reprocessing after 2000. The utilities are responsible for interim storage of spent fuel, and have formed joint companies to build and operate off-site surface facilities at

    The ARGE Weser (Arbeitsgemeinschaft zur Reinhaltung der Weser), which comprises representatives of the German states of Bremen, Hessen, Niedersachsen, Nordrhein-Westfalen and Thueringen, publishes a yearly water quality report based on the measuring programmes for the Weser river and for the Werra/Ulster rivers (the latter is a joint project of the states of Hessen and Thrueringen). The Weser river today has a water quality of II-III after the successful implementation of the 1989 action programme. Chloride concentrations were reduced as well, but efforts will still be made by communal authorities, agriculture, industry and administrative bodies if the goals set for 2000 are to be reached.     This (partial) project has the following aims: to describe comprehensively the possibilities for the recycling, or generation of energy from, waste with a high calorific value having undergone mechanical-biological conditioning; further, to formulate demands regarding the quality of the separated partial fractions. This basic study takes all relevant, commercial thermal processes into account (power plants, cement works, blast furnaces, etc.). Furthermore, the question is investigated of whether the thermal waste processing plants in the area of the Suedhessische Arbeitsgemeinschaft Abfall (SAGA) are suitable for waste utilization. An environmental compatibility statement is made, and the economic and legal boundary conditions are studied. (orig.)     Problem: The formulation of requirements is necessary to control the goals of a PACS project. Furthermore, in this way, the scope of functionality necessary to support radiological working processes becomes clear. Method: Definitions of requirements and specification are formulated independently of systems according to the IEEE standard 'Recommended Practice for Software Requirements Specifications'. Definitions are given in the Request for Information, specifications in the Request for Proposal. Functional and non-functional requirements are distinguished. The solutions are rated with respect to scope, appropriateness and quality of implementation. Results: A PACS checklist was created according to the methods described above. It is published on the homepage of the 'Arbeitsgemeinschaft Informationstechnologie' (AGIT) within the 'Deutsche Roentgengesellschaft' (DRG) (http://www.uni-marburg.de/mzr.agit). Conclusion: The checklist provides a discussion forum which should contribute to an agreement on accepted basic PACS functionalities. (orig.)

    Full Text Available In 2006 the journal "MEDIZIN – BIBLIOTHEK – INFORMATION" changed from the internet platform of the "Arbeitsgemeinschaft für Medizinisches Bibliothekswesen" (AGMB to the Open Access platform German Medical Science (GMS. The printed journal version ceased. Thitherto it was sent to all members of the AGMB. The editorial staff made a few arrangements in order to support acceptance and perceptibility of this journal, now issued electronic only. The published papers were included in the database of the Deutsches Bibliothekswesen (DABI and in the Directory of Open Access Journals (DOAJ Content. Furthermore current articles were introduced in MEDINFO, a weblog for medical librarians. The paper compares usage data trends from September 2005 to May 2008 for selected articles.

  16. Establishment of the Certification System "Gynaecological Dysplasia" in Germany.

    Gynaecological cancer centres have been established nationwide in Germany since 2008 according to the certification system of the German Cancer Society (Deutsche Krebsgesellschaft e. V. [DKG]) and the German Society for Gynaecology and Obstetrics (Deutsche Gesellschaft für Gynäkologie und Geburtshilfe e. V. [DGGG]). However, patient access to the certified gynaecological cancer centres is currently only possible through direct referrals. A longitudinal structure with the corresponding long-term documentation of both the high-grade precursors as well as the cancers does not exist as yet. According to the aims of the National Cancer Plan, a corresponding structure for the cancer entity "cervix carcinoma" should be established. The foundations for such a structure are appropriate diagnostic units that are responsible, after nationwide screening, for clarification according to guideline-conform principles. On the basis of the vote of the certification commission for gynaecological cancer centres under the chairmanship of the DKG, the Working Group for Gynaecological Oncology (Arbeitsgemeinschaft Gynäkologische Onkologie e. V. [AGO]), the Committee on Cervical Pathology and Colposcopy (Arbeitsgemeinschaft Zervixpathologie & Kolposkopie [AG-CPC]) and the DGGG the certification system for gynaecological dysplasia has been established. As a general principle, a distinction is made between the certification of a consulting practice for gynaecological dysplasia and a gynaecological dysplasia facility in order to integrate both outpatient and inpatient health-care facilities into the certification system. In analogy to the further catalogue of requirements from the DKG, quantitative and qualitative minimum numbers are demanded.     Full Text Available Production is a multi-step process involving many different articles produced in different jobs by various machining stations. Quality inspection has to be integrated in the production sequence in order to ensure the conformance of the products. The interactions between manufacturing processes and inspections are very complex since three aspects (quality, cost, and time should all be considered at the same time while determining the suitable inspection strategy. Therefore, a simulation approach was introduced to solve this problem.The simulator called QUINTE [the QUINTE simulator has been developed at the University of Dortmund in the course of two research projects funded by the German Federal Ministry of Economics and Labour (BMWA: Bundesministerium für Wirtschaft und Arbeit, the Arbeitsgemeinschaft industrieller Forschungsvereinigungen (AiF, Cologne/Germany and the Forschungsgemeinschaft Qualität, Frankfurt a.M./Germany] was developed to simulate the machining as well as the inspection.     Full Text Available Im Rahmen der Lehrveranstaltungs­reihe "Open Air" lud die Arbeitsgemeinschaft Güttl/Krisper/Röthler im WS 14/15 LehrerInnen zum Besuch des MOOCs "Gratis Online Lernen" der steirischen Plattform iMooX ein. Elf TeilnehmerInnen waren bereit, im Anschluss an die Teilnahme am Kurs ihre Erkenntnisse in ein Gemeinschaftswerk einfließen zu lassen, dessen Entstehung in der digitalen Werkstatt koordiniert wurde (Adobe Connect und Moodle. Aus der Überzeugung heraus, dass das gemeinsame Besuchen eines solchen Online-Kurses nur ein Teil eines Lernerlebnisses sein kann und das gemeinsame Erarbeiten für die Teilnehmerinnen und Teilnehmer ein zusätzlicher Schritt zum Überblicken der schier unendlichen Angebotslandschaft darstellt. Diese diskutierten, entwickelten, verwarfen und evaluierten interessante Lernangebote.   20. AGEB: preliminary 2008 energy balance for Germany

    According to preliminary calculations by the Arbeitsgemeinschaft Energiebilanzen (Energy Balances Working Group), the consumption of primary energy sources in Germany in 2008 rose by 1% from approx. 472 to nearly 478 million TCE. Especially the consumption of energy for space heating increased. The use of coal decreased. Electricity generation in nuclear power plants showed a clear increase. The share of renewable energies was determined by stable contributions by water power and an increase in wind power. In electricity generation, lignite was the most important energy source with approx. 150.0 billion kWh (23.5%), followed by nuclear power with 148.8 billion kWh (23.3%), hard coal with 128.5 billion kWh (20.1%), and natural gas with 83.0 billion kWh (13.0%). Among the renewable sources of energy, wind power rated 40.2 billion kWh (6.3%), followed by hydroelectric power with 27.0 billion kWh (4.2%). The other sources of renewable energy plus other sources and petroleum products contributed some 61.6 billion kWh (9.6%) to electricity generation. (orig.)

    E-mail is ideal for ad-hoc connections in teleradiology. The DICOM standard offers the possibility to append DICOM data types as a MIME attachment to any e-mail, thus ensuring the transmission of the original DICOM data. Nevertheless, there are additional requirements (e.g. protection of data privacy) which must be obeyed. Because of the lack of given standards which would grant interoperability as well as manufacturer independence, teleradiology has not been established in Germany until today. Therefore, the IT-Team (Arbeitsgemeinschaft fuer Informationstechnologie, rate at GIT) of the Radiological Society of Germany (Deutsche Roentgengesellschaft, DRG) set up an initiative to standardise telemedicine by using e-mail. Its members agreed that an e-mail-based variant would be the most practicable way to a communication solution - as easy to implement as to use. In their opinion, e-mail represents the smallest common denominator for a safe data interchange that would fulfill the legal advantages for telemedicine in Germany. (orig.)

    In a project supported by the Arbeitsgemeinschaft industrieller Forschungsvereinigungen e.V. (AiF) (research association) an oil burner for radiant tubes has been developed at the Department of Heat and Mass Transfer EST, RWTH Aachen. The capacity range of the burner is 20 to 40 kW, as combustible fuel oil (Heizoel Extra Leicht) is used. The radiant tube burner was investigated at furnace temperatures of 1000 C. Because of the complete vaporisation of the liquid fuel, the fuel-air-mixture burns in a blue flame. Despite of the fact that the combustion air was preheated to a temperature of 850 C, extremely low emissions were achieved. At a firing rate of 39 kW the NO{sub x}-emissions were at approx. 100 ppm (3% O{sub 2}), the CO-emissions were clearly below 25 ppm. By balancing the radiant tube temperature of {+-}15 K a very low value could be achieved, which is necessary for optimal heat transfer to the material to be heated. By the determination of the recirculating mass flows and validation of these values by computer-aided simulation, the understanding for the phenomena taking place could be clearly improved. The total recirculation ratio with a value of r{sub ges}{approx}1.5 explains the minimal NO{sub x}-emissions. (orig.)

    Magnetic resonance imaging has reached high acceptance in the diagnostic work-up of joint pathologies. Direct magnetic resonance arthrography extends the capabilities of conventional MR imaging and utilizes the natural advantages gained from joint effusion. The imaging technique has been applied in the shoulder, ellbow, hand, hip, knee and ankle joints. Besides technical aspects, this article wants to discuss indications for direct MR arthrography for clinical purposes. They are based on the recommendations of the 'Arbeitsgemeinschaft Muskuloskelettale Radiologie' of the German Radiology Society. This implies recognizing the inherent strengths and weaknesses of concurring imaging modalities as a basis for the distinguished clinical use of direct MR arthrography. Part I of this article describes the techniques of arthrography and MR imaging as well as magnetic resonance arthrography of the shoulder and elbow. At present, a final assessment of the technique at 3 T is not possible. However, it is anticipated that the possibilities of cartilage imaging may improve considerably so that MR arthrography at 3 T will replace CT-arthrography for detection of cartilage lesions especially in small joints.     The household survey took place in December 2002 in over 27 000 representatively selected German private households within the GfK Mail Panel. The total number of respondents can be characterised as very good at 20 325 or 75% of the gross sample, especially as some of the information required was relatively complex. This is especially true for the key questions on the amount of energy used and energy costs. The evaluation of the data resulted in an extrapolation of the energy consumption for the year 2002 on the one hand and, on the other, in analyses of energy related equipment and structural features as well as influencing variables on the energy consumption. A total energy consumption of 700 TWh resulted from this extrapolation for households in 2002 which can be split as follows: 18% for electricity, 38% for gas, 31% for fuel oil, 5% for district heating, 1% for coal and 7% for wood and other energy sources (Table2-2). Compared with the consumption shown by the Arbeitsgemeinschaft Energiebilanzen (2003), the extrapolations resulted in lower consumption figures in total and for all energy carriers with the exception of heating oil. However, the overall difference of almost 7% should not be rated as very large in view of the uncertainties connected with the survey and the extrapolation itself. (orig.)

    Full Text Available Despite major economic and organizational changes in the pharmaceutical industry, the Pharmaceutical Library Task Force (Arbeitskreis Pharma-Bibliotheken has been going strong in the Medical Library Working Party (Arbeitsgemeinschaft für medizinisches Bibliothekswesen – AGMB for the past 18 years. However, Task Force discussion topics have moved away from questions about spatial or staffing expansion or building up inventory, and toward themes such as outsourcing, training, technology scouting, and building up comprehensive copyright expertise. While actual physical stock is shrinking or has been eliminated entirely from many company libraries, the everyday working lives of company librarians are increasingly taken up with management of electronic media, in-house marketing and the associated user training, evaluation and use of new technologies, and clarification of complex copyright issues. Widespread know-how, in-depth knowledge of specialist areas (such as copyright, communication skills, networking, cost awareness, adaptability, willingness to learn new things, and demonstrating the value they add to the company are the factors that will secure the future of company librarians, whether with – or, in a pinch – without a physical library.

    For new reactor designs, the integrity of the containment must be ensured even in the case of severe accidents, like core-melt accidents, in order to avoid emergency measures outside the reactor plant. With view to this goal the German Interest Group AGIK performed theoretical investigations and industrial scale tests for proving the function and efficiency of innovative reactor safety elements. Within the COMAS project representative experiments on the spreading behaviour of prototypic corium on different substratum materials were carried out accompanied by conceptual work on novel core catcher designs (INKA). Inherent safety against spontaneous failure of the Prestressed Cast Iron Pressure Vessel with special view on its suitability for LWR conditions was investigated in the VGD project. The PACOS work showed the feasibility of passive cooling elements inside the containment. (orig.)     Due to the widespread industrial use of thermal plasmas in the field of joining, cutting and the application of coatings new challenges arise owed to the advent of new materials or the drive to reduce cost or improve quality. These challenges may be met by using technological innovations like innovative fast power supplies. In the presence of strong gas flows and a fixed cathodic attachment the anode attachment position is determined by an unstable balance between the drag force on the plasma column exerted by the gas and the Lorentz Force due to the system's magnetic field distribution, leading to a constant arc motion and arc voltage fluctuation. Thus by supplying a sufficiently high and steep current pulse a re-positioning might be initiated by a sudden change of the Lorentz Force thus an externally controlled movement of the arc would be possible. In wire arc spraying a pulsed current is imposed upon the DC supply of the wire arc system. It is observed that steep current increases tend to produce sudden current drops, indicating a jump of the arc. For a certain pulse frequency this pulsing leads to a controlled motion of the arc along the electrode surfaces. Coatings produced with this technology exhibited a lower porosity than DC sprayed coatings and a lower oxide content.     Based on case studies taken from the practice of energy consultants and manufacturers of energy saving products and equipment, united in the DESA (Deutsche Energiespar-Arbeitsgemeinschaft), reasons for rejecting energy saving measures in trade and industry have been analysed. The examples chosen for the analysis are cases, where amortisation time would have been limited to only a few years and a profit should have been possible to realize within the period of write down. Out of quite number of projects 22 cases have been chosen, where the energy saving potential is particular high and where economical competitive techniques have not been put into action instead. These projects, espacially the reasons for not realizing them, have been discussed in interviews with managing operators and suppliers. In the course of these interviews hindrances have been identified, which are common to all branches and techniques: Lack of information and motivation, remaning financial risks, but also uncertain scope data. On the other hand there are hindrances within the target groups, which are specific for certain objects and branches: Deficiencies in energy engineering knowledge and low equity capital disposal in small and medium size companies, unsufficient information on the level of the responsable management and not enough incentives for innovations in branches with a low fraction of relative energy costs. In the domain of public service additional obstacles are: Traditional views, technology refusal, separation of budgets for first and running costs or dual financing directives respectively. By means of 20 strategic points practical solution proposals have been made containing the necessary scope of time and responsbilities. These comprise as well legal as economic instruments. (orig.)     Full Text Available The journal impact factor (IF, which is published annually by the Institute for Scientific Information® (USA, is meanwhile in widespread use as a scientometric parameter for the evaluation of research and researchers in Germany and other European countries. The present article subjects the IF to critical analysis. It first deals with processes of production, transfer, and use of medical knowledge, because the IF intervenes in these processes on account of its reflexivity. Secondary effects of the IF resulting from its reflexivity are discussed with the focus on the level of the author, the journal and the medical discipline as well as on social knowledge processes in society. In addition, the extent to which the IF is appropriate for evaluating the quality of a specific article, of a journal or of individual and collective research achievements is discussed.     In many countries, percutaneous vertebroplasty became a widely accepted therapeutic option in osteoporotic and neoplastic fractures of the vertebral bodies which is employed in more and more patients. Under image guidance the method can be savely performed and provides a high success rate regarding stabilization of vertebral fractures and pain relief. Due to the increasing interest in vertebroplasty in Germany the German Radiological Society - Working Group on Interventional Radiology decided to produce guidelines for percutaneous vertebroplasty. This guidelines are based on the recent standard of knowledge and represent a guide for practical performance of this procedure. Based on the technical development of the method, indications and implementation of the method in multimodal therapy regimens may change over the next years. Therefore, this guidelines are not a rigid body of rules but a basis for an ongoing development adjusted to the scientific progress and the interdisciplinary discussion. (orig.) Die Methode kann unter bildgebender Kontrolle sehr sicher durchgefuehrt werden und ist durch eine hohe Erfolgsrate im Sinne der Wirbelkoerperstabilisierung und der Schmerzlinderung gekennzeichnet.   16. [Classification of periprosthetic shoulder fractures].

    The key targets in the treatment of periprosthetic humeral fractures (PHF) are the preservation of bone, successful bony consolidation and provision of a stable anchoring of the prosthesis with the major goal of restoring the shoulder-arm function. A substantial problem of periprosthetic shoulder fractures is the fact that treatment is determined not only by the fracture itself but also by the implanted prosthesis and its function. Consequently, the exact preoperative shoulder function and, in the case of an implanted anatomical prosthesis, the status and function of the rotator cuff need to be assessed in order to clarify the possibility of a secondarily occurring malfunction. Of equal importance in this context is the type of implanted prosthesis. The existing classification systems of Wright and Cofield, Campbell et al., Groh et al. and Worland et al. have several drawbacks from a shoulder surgeon's point of view, such as a missing reference to the great variability of the available prostheses and the lack of an evaluation of rotator cuff function. The presented 6‑stage classification for the evaluation of periprosthetic fractures of the shoulder can be considered just as simple or complex to understand as the classification of the working group for osteosynthesis problems (AO, Arbeitsgemeinschaft für Osteosynthesefragen), depending on the viewpoint. From our point of view the classification presented here encompasses the essential points of the existing classification systems and also covers the otherwise missing points, which should be considered in the assessment of such periprosthetic fractures. The classification presented here should provide helpful assistance in the daily routine to find the most convenient form of therapy. PMID:26992712

  17. The role of radiotherapy for interdisciplinary treatment of the malignant melanoma

    As compared to new foreign studies, the recent German literature highlights the hyperthermal infusion therapy but gives only poor information about the progress of local radiotherapy for stage I and II. Above all, the endolymphatic radionuclide therapy is only seldom mentioned. With our own patients (n = 106), we could achieve a five-year cure rate of more than 73% for stage I. Almost 90% of the patients had a Clark level of III to V, 65% had a level of IV. With respect to the relatively simple method and the low rate of complications, one can no longer ignore the identical results of the Deutsche Arbeitsgemeinschaft with 338 cases hitherto. For stage II, fast electrons and, eventually, neutrons can be applied according to the situation; a combination with local hyperthermia and chemical radiosensitizers is being tested. Pilot experiments made in London (Catterall) with neutrons have shown encouraging results, and the observations made by us at the cyclotron of Essen induces us to reexaminate patients with recurrent tumors and/or lymph node metastases. With regard to the fully sufficient radiosensibility of many melanomas proved by recent radiobiologic research, fractionation schemes other than the usual daily irradiation have become particularly important. The exclusive superficial irradiation in case of melanoma is a historical method which is only applicable for small local recurrences and in-transit metastases. The more recent radiotherapeutic possibilities mentioned in this report, however, are worth further consideration in the future clinical and therapeutical research.     Full Text Available Die Einführung der elektronischen Gesundheitsakte, kurz ELGA, in Österreich ist ein seit längerer Zeit kontrovers diskutiertes Thema. Gerade in den letzten Monaten waren vermehrt Lebenszeichen der ELGA in der Presse zu finden, deren Umsetzung Teil der aktuellen Regierungsvereinbarung ist. Zudem wurden in vielen Gesprächen im Rahmen des Studiums immer wieder Vorwürfe geäußert, dass die Vorgaben seitens der Arbeitsgemeinschaft ELGA (ARGE ELGA zu sehr techniklastig seien, während die Forderungen der einzelnen zukünftigen Benutzer nicht oder zu wenig Beachtung finden. Im Rahmen der vorliegenden Arbeit wurde erarbeitet, wie sehr sich wirklich die Vorgaben seitens der ARGE ELGA auf den Bereich Technik konzentrieren bzw. welche Benutzeranforderungen bereits definiert wurden. Im nächsten Schritt wurden mittels eines problemzentrierten Interviews verschiedene Benutzergruppen der geplanten ELGA zu deren funktionalen Benutzeranforderungen befragt. Die ausgewählten Stakeholder waren: Ärzte, Apotheker, Soziale Dienste und Vertreter von Selbsthilfeorganisationen.Aus den durchgeführten 29 Interviews wurde im Anschluss eine Auswertung von funktionalen Benutzeranforderungen für die bislang beschriebenen Kernanwendungen der ELGA (Portal, e-Arztbrief, e-Befund Labor/Radiologie und e-Medikation erstellt. So wurde am Vordringlichsten die Einführung einer elektronischen Medikamentenübersicht bzw. der e-Impfpass gewünscht. Aber auch die Möglichkeit zur Präsentation der Selbsthilfe im Rahmen des ELGA-Portals war ein Punkt mit hoher Zustimmung. Demgegenüber wurde der Vorschlag des Versteckens von Dokumenten bzw. der Online-Kontakt mit dem behandelnden Arzt deutlich abgelehnt. Als Ergebnis wird zusammengefasst, dass derzeit keine ausreichende Definition von funktionalen Benutzeranforderungen für die elektronische Gesundheitsakte verfügbar ist.   19. Consensus Guideline Based Therapeutic Drug Monitoring (TDM) in Psychiatry and Neurology.

    Therapeutic drug monitoring (TDM) is a valuable tool for tailoring the dosage of the prescribed medication(s) to the individual pharmacokinetic characteristics of a patient. In psychiatry and neurology, however, proven evidence that TDM should be used for treatment with the multiple neuropsychiatric medications is restricted to few compounds. Well-designed clinical trials on medical and economic benefits of TDM are rare. The use of TDM is limited in most countries to few antiepileptics, especially carbamazepine, phenobarbital and phenytoin, some mood stabilizers, especially lithium and valproic acid, some antidepressants, especially tricyclic antidepressants and some antipsychotics, primarily clozapine because these drugs have a narrow therapeutic index. On the other hand, specific indications and distinct problems can make TDM most useful for individualized pharmacotherapy with almost any neuropsychiatric drug. Potential benefits of TDM can, however, only be reaped if the method is adequately integrated into the clinical treatment process. The TDM expert group of the Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie (AGNP) issued consensus guidelines for the best practice of TDM in psychiatry and neurology. A first version was published in 2004. These guidelines were extended in 2011 and are actually updated (see: www.agnp.de). Exemplified by single cases it is shown here how to use TDM consensus guidelines for problem solving in psychiatry and neurology. Studies on depressed patients give evidence for tricyclic antidepressants, venlafaxine and citalopram that TDM could become a standard of care in psychiatry and neurology. There is potential to accelerate improvement. Reducing phases of suffering will not only have medical benefits for the patients but also an impact on costs for the health system which needs to be clarified by controlled studies. PMID:26511090

  20. Pebble bed modular reactor (PBMR)

    In 1993, the pebble bed modular reactor (PBMR) was identified by ESKOM, the electric utility of South Africa, as a leading option for the installation of new generating capacity to their electric grid. This innovative nuclear power plant incorporates a closed cycle primary coolant system utilizing helium to transport heat energy directly from the modular pebble bed reactor to a recuperative power conversion unit with a single-shaft turbine/compressor/generator. This replacement of the steam cycle that is common in present nuclear power plants (NPP) with a direct gas cycle provides the benefits of simplification and a substantial increase in overall system efficiency with the attendant lowering of capital and operational costs. Although the historical development of this plant is interrelated to other types of high temperature gas cooled reactors (HTGRs), the principle focus herein is on the pebble bed (spherical) fuel element type reactor. The long-term development of this reactor type began in Germany by the KFA Nuclear Research Center (now FZJ). Two pebble bed plants were constructed in Germany, the 46 MW(th)/15 MW(e) Arbeitsgemeinshaft Versuchsreaktor (AVR) and the 750 MW(th)/296 MW(e) thorium high temperature reactor (THTR-300). Basically, these steam/electric plants validated the temperature and fission product retention capabilities of the ceramic (TRISO) coated fuel particle and the safety characteristics of the HTGR. Most notable of the operational achievements was with the AVR in sustaining longterm operation at an average core outlet temperature of 950 deg. C, and in demonstration of safety such as extended loss of forced cooling on the core.     In addition to the two surgeons Hans Killian and Helmut Schmidt the pharmacologist Hellmut Weese was elected honorary member of the German Association of Anaesthesiology at the organisation's founding congress in 1953. Especially the younger colleagues are quite unfamiliar with his name, although a "Hellmut Weese commemorative" lecture is annually held at our national association's gathering. While it is widely known that his name is intimately connected with the introduction of evipan anaesthesia during the early thirties, only few people know about his extensive research on the therapy of shock that lead to the development of the plasma-expanding substance "Periston". Moreover, his pharmacological investigations on sympathomimetic amines and long-term studies on cardiac glycosides which he published in a well recognised monograph in 1936 have fallen into oblivion. Since he had not collaborated with Nazi regime he was able to start teaching at the University of Cologne again in the fall of 1945. One year later he became head of the Department of Pharmacology at the Medical Academy of Düsseldorf. There he started--together with colleagues of similar ideas and opinions--holding lectures on clinical anaesthesia at the beginning of the winter term 1948/49. People who attended his lectures later claimed that these were the "best lesson ever". At the same time the call for specialists for anaesthesiology that was commonly heard in Anglo-American countries could not be ignored in Germany any longer. Weese, who was already an honorary member of the International Anesthesia Research Society at the Association's congress in New York in 1938, realised the lack of trained physicians. Thus he fought enthusiastically for the idea of producing specialists and as a member of a committee for questions concerning anaesthesia played in important part in founding the "Deutsche Arbeitsgemeinschaft für Anästhesiologie".     Background For some specific comminuted unstable intra-articular fracture,the plaster cast can not maintain the alignment of the articular surface effectively.The aim of this study was to evaluate the clinical effects of distal radius fracture treated with open reduction and internal plate fixation retrospectively.Methods From January 2002 to March 2010,539 cases of distal radius fracture were treated with open reduction and internal fixation,including 184 males and 355 females aging 21-72 years (mean 57 years).Fractures were caused by falling to the ground in 459 cases,by traffic accident in 62 cases and by athletic injuries in 18 cases.Of 539 cases,there were 523 cases of closed fracture and 16 cases of open fracture.According to Arbeitsgemeinschaft fur Osteosynthesefragen (AO) standards of classification,there were 14 cases of A2 type,22 of A3 type,18 of B1 type,24 of B2 type,62 of B3 type,91 of C1 type,162 of C2 type and 146 of C3 type.The time from injury to operation was 1-16 days (mean 5 days).All patitents received open reduction and internal plate screw fixation.Forty-seven patients with bone defect were given 6-15 g autologous ilium and 75 cases were given 5 ml calcium sulphate artificial aggregate after reduction.Results All incisions healed by first intention after operation.Patients were followed up for 15 to 32 months postoperatively (mean 22 months).The fractures healed within 10-18 weeks after operation (mean 12 weeks).During the last follow-up,the mean palmar tilt was (7.0±0.9)° and the mean ulnar variance was (21.0±4.2)°,showing significant difference when compared with preoperation ((-5.0±1.2)° and (8.0±3.8)°).The radial heights were not abbreviated.According to Gartland and Werley assessment system,the results were excellent in 314 cases,good in 163 cases,fair in 46 cases,and poor in 16 cases 12 weeks after operation,the excellent and good rate was 88.5%.Conclusions The clinical effect of distal radius fracture treated with open

  3. 2005 primary energy consumption in Germany

    According to preliminar calculations by the Arbeitsgemeinschaft Energiebilanzen (Working Party on Energy Balances, AGEB), the consumption of an aggregate 486 million TCE of primary energy resources in Germany last year was 1.3% below the level of the year before. Energy consumption was influenced by both the high level of prices and the development of the economy. Hardly any influence was attributable to the level of temperatures, which was largely unchanged compared to the figure of the year before. Oil consumption in 2005 in Germany dropped by nearly 2% to 174.8 million TCE. On the whole, oil with its 36% share in the energy balance remained by far the most important energy resource in Germany. Natural gas consumption of 110.4 million TCE was at the level of the year before. Its share in the primary energy balance rose slightly to 22.7%. Hard coal, because of lower use in power plants and the decline in iron making, showed a 4.6% drop in consumption to 62.8 million TCE. In this way, hard coal contributed 13% to total energy consumption. Lignite consumption dropped by 3.2% to 54.4 million TCE as a result of lower deliveries to power plants. Its 11.2% share in the total consumption of primary energy continued to make lignite the most important domestic energy resource. More than 90% of the lignite produced is used for electricity generation. The contribution to primary energy consumption of nuclear power dropped by more than 2% to 60.7 million TCE. Hydroelectric plants and wind power plants increased their contribution by 3.6%. The contribution to primary energy consumption made by all renewable energy resources rose to 4.6%. AGEB evaluates statistics of all areas of the power economy on the basis of standard criteria in order to combine these data in a comprehensive picture.   4. Deformation integrity monitoring for GNSS positioning services including local, regional and large scale hazard monitoring - the Karlsruhe approach and software(MONIKA)

    GNSS-positioning services like SAPOS/ascos in Germany and many others in Europe, America and worldwide, usually yield in a short time their interdisciplinary and country-wide use for precise geo-referencing, replacing traditional low order geodetic networks. So it becomes necessary that possible changes of the reference stations' coordinates are detected ad hoc. The GNSS-reference-station MONitoring by the KArlsruhe approach and software (MONIKA) are designed for that task. The developments at Karlsruhe University of Applied Sciences in cooperation with the State Survey of Baden-Württemberg are further motivated by a the official resolution of the German state survey departments' association (Arbeitsgemeinschaft der Vermessungsverwaltungen Deutschland (AdV)) 2006 on coordinate monitoring as a quality-control duty of the GNSS-positioning service provider. The presented approach can - besides the coordinate control of GNSS-positioning services - also be used to set up any GNSS-service for the tasks of an area-wide geodynamical and natural disaster-prevention service. The mathematical model of approach, which enables a multivariate and multi-epochal design approach, is based on the GNSS-observations input of the RINEX-data of the GNSS service, followed by fully automatic processing of baselines and/or session, and a near-online setting up of epoch-state vectors and their covariance-matrices in a rigorous 3D network adjustment. In case of large scale and long-term monitoring situations, geodynamical standard trends (datum-drift, plate-movements etc.) are accordingly considered and included in the mathematical model of MONIKA. The coordinate-based deformation monitoring approach, as third step of the stepwise adjustments, is based on the above epoch-state vectors, and - splitting off geodynamics trends - hereby on a multivariate and multi-epochal congruency testing.     Full Text Available Empfehlungen zur Modifizierung des Lebensstils einschließlich körperlichen Trainings sind die Basis der Therapie bei allen kardiovaskulären und metabolischen Erkrankungen. Der günstige Einfluß von regelmäßigem körperlichem Training auf die Morbidität und Mortalität von Herz-Kreislauf-Erkrankungen ist ausreichend belegt. Körperliches Training gehört deshalb mit zu den primären Behandlungsempfehlungen bei der arteriellen Hypertonie, der Hyperlipidämie, des Übergewichts und des Diabetes mellitus – wenn möglich bereits vor, aber auch während einer medikamentösen Therapie. Um möglichst vielen Betroffenen ein gezieltes, wirksames und nach modernen Kriterien der medizinischen Trainingslehre aufgebautes körperliches Training zu ermöglichen, müssen diese in der Lage sein, das Training zu Hause selbständig durchzuführen und in den Alltag zu integrieren. Die Herausforderung besteht darin, die Patienten in der richtigen Anwendung von körperlichem Training zu schulen. Durch ein persönliches Einführungsgespräch oder die alleinige Aushändigung schriftlicher Unterlagen ist es wahrscheinlich nicht möglich, ausreichendes Wissen um die positiven Wirkungen von körperlichem Training zu vermitteln und Patienten nachhaltig zu motivieren, dieses in ihren gesunden Lebensstil einzubinden. Es erscheint vielmehr notwendig, die Patienten so lange unter enger Kontrolle zu halten, bis eine meßbare und subjektiv wahrnehmbare Leistungssteigerung sowie eine Besserung der gesundheitsbezogenen Lebensqualität zu einer Festigung der Selbstmotivation führen.   6. Long-term creep properties, including irradiation effects, of DIN 1.4948 steel from SNR-300 primary components

    Since 1968, the ''Arbeitsgemeinschaft warmfeste Staehle in VDEH'', sponsored by KfK/PSB, performs creep tests on austenitic stainless steel DIN 1.4948. Wrought materials both in the solution-annealed condition and in weldments are investigated. The DIN 1.4948 steel was finally chosen for the permanent primary structures of the SNR-300, such as the reactor vessel, the grid plate and the primary piping. The chemical specification of DIN 1.4948 steel has been narrowed for application in the SNR-300. The long-term programme at KfK on SNR-300 heats has reached times to rupture of up to 90,000 h for parent metal. At 823 K, the operating temperature of the SNR-300, the mean creep rupture strengths of standard DIN 1.4948 and SNR-300-grade DIN 1.4948 are equal, but the scatter of data for standard DIN 1.4948 parent metal is twice as large as that for SNR-300-grade parent metal. The strength of SNR-300 weldments falls within the scatter band for parent metal. At times to rupture of more than 104 h, all parent metals have ductilities of between 10% and 20%, whereas the welded joints show ductilities below 1%. The neutron irradiation effects on SNR-300-grade DIN 1.4948 heats and weldments have been studied with times to rupture of up to 10,000 h in the range from 723 K to 923 K. After irradiation the creep strength is reduced to values below the minimum stress rupture values according to the ASME B + PV Code. The ductility values of parent metal after irradiation are in the range of 4% to 10%. The ductility of welded joints is sometimes below 1%. With increasing Larson-Miller parameter the reduction in creep strength decreases. Therefore, long-term post-irradiation creep tests have been started at KfK and ECN in order to verify experimentally the creep strength reduction factor due to irradiation, for times to rupture of between 104 and 5x104 h.     Background Few data on ankle fractures in China from large multicenter epidemiological and clinical studies are available.The aim of this research was to evaluate the epidemiological features and surgical outcomes of ankle fractures by reviewing 235 patients who underwent ankle fracture surgery at five hospitals in China.Methods This study included patients who underwent ankle fracture surgery at five Chinese hospitals from January 2000 to July 2009.Age,gender,mechanism of injury,Arbeitsgemeinschaft für Osteosynthesefragen (AO) fracture type,fracture pattern,length of hospital stay and treatment outcome were recorded.Statistical analyses were conducted using SPSS software.The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale,visual analogue scale (VAS),and arthritis scale were used to evaluate outcome.Results Of 235 patients with ankle fractures,105 were male with an average age of 37.8 years and 130 were female with an average age of 47.3 years.The average follow-up period was 55.7 months.There were significant differences in the ratios of patients in different age groups between males and females,and in mechanisms of injury among different age groups.There were also significant differences in the length of hospital stay among different fracture types and mechanisms of injury.In healed fractures,the average AOFAS ankle-hindfoot score was 95.5,with an excellence rate of 99.6%,the average VAS score was 0.17,and the average arthritis score was 0.18.Movement of the injured ankle was significantly different to that of the uninjured ankle.There were no significant differences between AO fracture types,fracture patterns or follow-up periods and AOFAS score,but there were some significant differences between these parameters and ankle joint movements,pain VAS score and arthritis score.Conclusions Ankle fractures occur most commonly in middle-aged and young males aged 20-39 years and in elderly females aged 50-69 years.The most common mechanisms of

  8. Outcome of Radial Head Arthroplasty in Comminuted Radial Head Fractures: Short and Midterm Results

    Background: Comminuted radial head fractures are often associated with secondary injuries and elbow instability. Objectives: The aim of this retrospective study was to evaluate how well the modular metallic radial head implant EVOLVE® prosthesis restores functional range of motion (ROM) and stability of the elbow in acute care. Patients and Methods: Eighty-five patients with comminuted radial head fractures and associated injuries received treatment with an EVOLVE® prosthesis between May 2001 and November 2009. Seventy-five patients were available for follow-up. On average, patients were followed for 41.5 months (33.0: 4.0 - 93.0). Outcome assessment was done on the basis of pain, ROM, strength, radiographic findings, and functional rating scores such as Broberg and Morrey, the Mayo elbow performance index (MEPI), and disabilities of the arm, shoulder and hand (DASH). Our study is currently the largest analysis of clinical outcome of a modular radial head replacement in the literature. Results: Overall, there were 2 (2.7%) Mason II fractures, 21 (28%) Mason III fractures, and 52 (69.3%) Mason IV fractures. Arbeitsgemeinschaft fur osteosynthesefragen (AO) classification was also determined. Of the 85 patients in our study, 75 were available for follow-up. Follow-up averaged 41.5 months (range, 4 - 93 months). Average scores for the cohort were as follows: Morrey, 85.7 (median 90.2; range 44.4 - 100); MEPI, 83.3 (85.0; 40.0 - 100); and DASH 26.1 points (22.5; 0.0 - 75.8). Mean flexion/extension in the affected joint was 125.7°/16.5°/0° in comparison to the noninjured side 138.5°/0°/1.2°. Mean pronation/supination was 70.5°/0°/67.1° in comparison to the noninjured side 83.6°/0°/84.3°. Handgrip strength of the injured compared to the non-injured arm was 78.8%. An important aim for promoting clinical research and patient care in radiation oncology is to further professionalize planning and implementation of clinical trials in this field. Correct assessment, at an early stage, whether a trial needs to be approved by the BfS may reduce unneccesary costs and reduce the time needed for the approval procedure for those trials which need to be assessed by the BfS. [German] Bei Anwendung ionisierender Strahlung in der medizinischen Forschung ist in klinischen Studien im Bereich der Radioonkologie in Deutschland eine Genehmigung durch das Bundesamt fuer Strahlenschutz (BfS) notwendig. Bei der Abgrenzung zwischen ''Heilkunde'' und ''medizinischer Forschung'' kommt es bei radioonkologischen Studien immer wieder zu widerspruechlichen Einschaetzungen. Ziel ist es, eine Handreichung fuer Antragsteller, im Beantragungsprozess involvierte Personen und Institutionen sowie Ethikkommissionen zu liefern. Die Inhalte sind das Resultat eines von der Deutschen Gesellschaft fuer Radioonkologie (DEGRO) zusammen mit der Arbeitsgemeinschaft Radioonkologie (ARO) der Deutschen Krebsgesellschaft, der Deutschen Gesellschaft fuer Medizinische Physik (DGMP) und dem Deutschen Konsortium fuer Translationale Krebsforschung (DKTK) veranstalten Workshops zur klinischen Forschung in der Radioonkologie im Oktober 2013 sowie des nachfolgenden Abstimmungsprozesses mit dem BfS. Die Abgrenzung zwischen Heilkunde und medizinischer Forschung erfolgt anhand der Frage, ob es sich bei der therapeutischen Anwendung um ein nach Art und Umfang klinisch anerkanntes Strahlentherapieverfahren handelt oder nicht. Zur Einordnung werden die bestmoegliche Evidenz aus klinischen Studien sowie Stellungnahmen wissenschaftlicher Fachgesellschaften herangezogen
