The literature is littered with reports presenting the proportion of patients admitted with ischaemic stroke receiving thrombolysis. This gives some measure of activity and the efficiency of prehospital and hospital triage. Some stroke centres have reported rates of thrombolysis as high as 20%, but without knowing the population that such services serve, these proportions do not indicate how well or equitably the treatment is being delivered. Here, we explore various metrics to monitor our efforts to provide equitable access to stroke thrombolysis in Lothian, a region in the South East of Scotland. PMID:21086712

Intracranial atherosclerosis is a major cause of ischemic stroke, and depending on the studied population, it accounts for 8%-15% of all strokes that are due to cerebral atherosclerosis. The prognosis of patients with symptomatic intracranial stenoses seems to depend on the location and extent of intracranial atherosclerosis. Spastic equinovarus foot deformity commonly occurs after stroke or head trauma. Management by split anterior tibial tendon transfer to the lateral border of the foot was chosen by the authors. The purpose of that work was to assess the reliability of this method to improve patient's functional autonomy and investigate the relationship between the initial clinical examination findings and the quality of the functional results. 84 feet, operated in 82 patients out of 130, of mean age 46 years, were reviewed at a mean follow-up of 65 months. Bilateral breast carcinoma accounts for approximately 5% of all patients with breast cancer, while neuroendocrine breast carcinomas comprise less than 5% of invasive breast carcinomas. In addition, most patients with breast neuroendocrine carcinomas are older. Therefore, bilateral primary breast neuroendocrine carcinoma at a young age is extremely rare. We herein report bilateral neuroendocrine carcinoma of the breast in a 29-year-old woman who underwent bilateral lumpectomy with the initial symptom of bilateral nipple discharge. This report describes a technique of inserting an implantable venous access port (portacath) through a thrombosed and occluded vein employing a pre-existing peripherally inserted central catheter (PICC) as the route of access. The PICC was used as a conduit for venous access in a way that has not been described previously in the literature. This procedure was performed in a young patient with cystic fibrosis in an effort to prevent the use of his virgin contralateral veins, which might be used in the future.

We report here the imaging findings of a rare case of a lethal form of Gorham disease in a young female patient. Multimodality imaging findings over 13 year-follow-up demonstrated progressive wide spread skeletal and soft tissue abnormalities with permeative osteolysis, pathological fractures and severe skeletal deformities. Unusual extensive osseous and soft tissue pneumatosis was illustrated on cross-sectional studies. The progressive nature of this form of Gorham disease and the subsequent complications eventually culminated in patient's death. (orig.)

To determine MTRs for normal structures and benign diseases in the breast two-dimensional magnetization transfer imaging was performed in 62 patients and in 3 young female volunteers. With regard to the MTRs of measurements in the normal breast tissues, fat tissues which is close to simple cysts in MTRs show little transfer of longitudinal magnetization. MTRs of the muscles was 15.15{+-}6.22%, which exceeded those of breast parenchyma. The breast parenchyma didn`t show the change of MTR value due to the difference of patient age and due to variable amount of fat and fibrous tissues. Breast parenchyma in the two young volunteers clearly showed biphasic change of MTR values in accordance with the menstrual cycle; little transfer value was due to hydration in the postovulatory period and high transfer value was due to dehydration in the preovulatory period. Posttraumatic tibia valga is a well-recognized complication following fracture of the upper tibial metaphysis in young children. We present a case of a child who developed a valgus deformity following fracture of the proximal tibia and fibula in which quantitative bone scintigraphy at 5 months after injury demonstrated increased uptake at the proximal tibial growth plate with proportionally greater uptake on the medial side. This finding suggests that the valgus deformity in this patient was due to a relative increase in vascularity and consequent overgrowth of the medial portion of the proximal tibial physis.

Magnetic resonance imaging (MRI) in stroke makes it possible to visualize the initial infarct in cases of acute cerebral ischemia. Perfusion MRI serves to determine which tissues are additionally at risk of infarction due to persistent hypoperfusion. MRI also allows those examiners with limited experience to reliably confirm an infarct. The most important differential diagnosis of cerebral ischemia, intracerebral hemorrhage, can likewise be recognized with certainty using MRI. Although diffusion and perfusion MRI only demonstrate the pathophysiology of cerebral ischemia approximately, the method is suited for identifying those patients who would profit from reperfusion therapy. Cerebral blood flow (CBF) and cerebrovascular reserve capacity (CRC) were measured by stable xenon computerized tomography (Xe-CT) and acetazolamide test in 15 patients with cerebrovascular disease before and after extracranial-intracranial (EC-IC) bypass surgery for minor stroke, reversible ischemic neurological deficit or transient ischemic attack. All had angiographically shown occlusive lesions of the major arterial trunk. In the present series, global analysis showed that the bypass did not increase the resting rCBF, but did increase the rCRC. We divided the patients into four groups according to the preoperative resting rCBF and rCRC. All 3 patients with normal resting rCBF and reduced rCRC showed postoperative improvement of rCRC. Young children are at increased risk for valproic acid (VPA) hepatotoxicity. Urinary organic acid profiles, as a surrogate of mitochondrial function, were obtained in children 1.9 to 17.3 years of age (n = 52) who were undergoing treatment with VPA for seizure disorders. Age-matched patients receiving treatment with carbamazepine (CBZ; n = 50) and healthy children not undergoing treatment (n = 22) served as controls. Ewing's sarcoma is the most common malignant bone tumour occurring in children and adolescents and exists in two different clinico pathological entities: osseous Ewing's sarcoma (OES) and extraosseous Ewing's sarcoma (EES). Five cases of primary epidural EES are described, which presented with non-specific symptoms leading to a long diagnostic delay. The median age at diagnosis was 22 years (range 13-36 years). The median diagnostic delay was 3 months. All patients had one or more neurological deficits. All underwent surgical exploration with a laminectomy and partial resection followed by adjuvant radiotherapy to a dose of 46-50 Gy and chemotherapy with VAC (vincristine, adriamycin and cyclophosphamide) alternating with ICE (ifosphamide, cisplatin and etoposide) for at least six cycles. Laser-Doppler anemometry was used to investigate mean flow and turbulence fields in a direct injection diesel engine, motored at speeds of 1500, 2250 and 3000 rev/min. Both cycle-resolved and ensemble-averaged measurements were made in the upper part of the cylinder during the inlet and compression strokes. The cycle-resolved measurements in the inlet stroke are discussed in this paper.

Objective: To assess dynamic MRI with Gd-DTPA enhancement for evaluating inflammatory changes in the subacromial bursa. Design and patients: We detected the signal intensity changes in dynamic MRI of the subacromial bursa, and confirmed these macroscopically by arthroscopy and histologically. The signal intensity was measured using built-in software, and the enhancement ratio (E ratio) was calculated from dynamic MR images. In addition, as a parameter of the rate of the increase in the signal intensity from 0 to 80 s, the mean increase per second in the E ratio was obtained as the coefficient of enhancement (CE). The solid-pseudopapillary tumor (SPT) of the pancreas is a rare type of exocrine pancreatic neoplasm. SPT predominantly affects young women and female children, and is usually discovered incidentally. This tumor is generally benign with a low incidence of malignancy. A 13-year-old girl was admitted to the hospital with a few weeks' history of mild abdominal pain and jaundice. On physical examination, there was no palpable mass. The laboratory tests showed increased SR, CRP, high bilirubin, amylase and lipase serum levels. Ultrasound imaging revealed a solid lesion in the region of the pancreatic head. On MRI, precise tumor localization in the head of the pancreas with pancreatic duct dilatation and compression of the common bile duct were visualized. Pancreaticoduodenectomy and cholecystectomy was performed with good clinical outcome. A novel compact CT-guided intensity modulated proton radiotherapy (IMPT) system is described. The system is being designed to deliver fast IMPT so that larger target volumes and motion management can be accomplished. The system will be ideal for large and complex target volumes in young patients. The basis of the design is the dielectric wall accelerator (DWA) system being developed at the Lawrence Livermore National Laboratory (LLNL). The DWA uses fast switched high voltage transmission lines to generate pulsed electric fields on the inside of a high gradient insulating (HGI) acceleration tube. High electric field gradients are achieved by the use of alternating insulators and conductors and short pulse times. In this paper compression and power stroke cycles for a 4 stroke cycle spark ignition engine modified by extending the connecting rod to simulate purely sinusoidal piston motion are analyzed over a range of operating speeds and are compared with those of a similar conventional engine. Heat release rate is estimated for both engines using a simple Wiebe function with the functional parameters found via a simplex curve fitting method is used in conjunction with experimental pressure curves. It is shown that the functional parameters which represent the combustion and duration of fuel burn are slightly larger over the range of operation in the sinusoidal engine while the shape factor remains largely the same.

As part of the EPRI Motor-Operated Valve (MOV) Performance Prediction Methodology, a System Flow Model (SFM) has been developed to determine the differential pressure (DP) across MOVs as they are stroked in typical power plant systems. Input to the SFM includes valve flow characteristics (such as flow coefficient) and system characteristics (such as pump head/flow curve). The primary output is differential pressure across the valve at each stroke position. The SFM was validated by comparison with test data from the EPRI Flow Loop Test Program. In this study, we calculated DP using SFM and performed DP test for four MOVs. Compared between calculated DP and test DP of four MOV, respectively good agreements are found. This paper presents new images and spectroscopy of NGC 34 (Mrk 938) obtained with the du Pont 2.5-m and Baade 6.5-m telescopes at Las Campanas, plus photometry of an HST archival V image. This Mv = -21.6 galaxy has often been classified as a Seyfert 2, yet recently published infrared spectra suggest a dominant central starburst. We find that the galaxy features a single nucleus, a main spheroid containing a blue central disk, and tidal tails indicative of two former disk galaxies. These galaxies appear to have completed merging. The remnant shows three clear optical signs that the merger was gas-rich ("wet") and accompanied by a starburst: (1) It sports a rich system of young star clusters, of which 87 have absolute magnitudes -10.0 > Mv > -15.4. A total of 10 samples disks of 2024-T3 aluminum and 4130 ferrous steel were exposed to rocket-triggered lightning currents at the Kennedy Space Center test site in Florida during the summer of 1990. The experimental configuration was arranged so that the samples were not exposed to the preliminary streamer, wire-burn, or following currents that are associated with an upward-initiated rocket-triggered flash but which are a typical of naturally initiated lightning. Return-stroke currents and continuing currents actually attaching to the sample were measured, augmented by close-up video recordings of approximately 3 feet of the channel above the sample and by 16-mm movies with 5-ms resolution. A method, which estimates the lightning performance of high voltage transmission lines based on the Monte-Carlo simulation technique, is described in this paper. The average number of faults which occur in a transmission line, dividing them in single-phase and three-phase faults, as well as the average grounding resistances of the transmission lines are calculated. The method is applied, on several operating Greek transmission lines, showing good correlation between predicted and field observation results. The proposed method can be used as a useful tool in the design of electric power systems, aiding in the right insulation dimensioning of a transmission line.(author)

The Northwest Foster Care Alumni Study examined the relation between experiences in foster care and depression among young adults who spent at least a year in foster care as adolescents. Results indicate that preparation for leaving foster care, nurturing supports from the foster family, school stability, access to tutoring, access to therapeutic service and support, and a shorter and more stable placement history were associated with a lower probability of depression in young adulthood. Results provide evidence that suggest ways to improve practice to decrease rates of depression among alumni of care. (Contains 2 tables.)

Two natural gas/diesel dual fuel conversion systems for the Detroit Diesel 6V-71 transit bus engine were described. The basic concept of a dual fuel engine was to operate a diesel engine with both liquid fuel injection and a lean premixed gas mixture. The engine achieved high efficiency due to the high compression ratio typical of the diesel engine. Test results were obtained using three different calibrations on one engine. The objectives were to match existing engine performance with improved emissions and better overall fuel economics. Results showed that overall, engine conversion has both economic and emissions advantages over a straight diesel engine. However, the durability and in-use applicability of the system remains to be demonstrated with an in-bus demonstration project.. 3 tabs., 4 figs.

The calculations of Gay et al. of the terminal velocities of charged hydrormeteors in the presence of electric fields have formed the basis of computations of the charging current density J flowing through a thunder-cloud as a result of the operation of a precipitative mechanism of cloud electrification. Values of J were calculated for a range of values of field strength E, precipitation rate pO, precipitation content L, cloud water content C, charge distribution, total separated charge, and the fraction of the small particles that have undergone a charging event.It is found that the estimated field required for the initiation of a lightning stroke (3.5 kV cm1 can be achieved only over a narrow range of conditions. The ease with which precipitative mechanisms can produce breakdown fields is considerably increased, however, if account is taken of spatial inhomogenities in the field.

In seeking ways to reduce emissions from two-stroke locomotive type engines, the Navy has evaluated dual-fuel conversions operating on a compression ignition cycle, using up to 94% natural gas and 6% diesel pilot fuel. The Navy has conducted an evaluation and test program under the direction of Dr. Normnn L. Helgeson, at the Naval Facilities Engineering Service Center in Port Hueneme, California. Of the Navy`s many diesel engines, those installed in its MUSE (mobile utility support equipment) units for temporary electrical power were the first Navy off-road engines to be affected by emissions regulations. Most of the units are powered by the EMD 645 engine, and when burning diesel fuel do not meet the emission requirements in many areas of the country. This paper discusses the changes and results of the conversion and shakedown tests.

Counter rotating longitudinal vortices produced by winglet in a channel are known to enhance heat transfer. In the present investigation the flow structure and heat-transfer enhancement by a winglet pair of non-zero thickness has been studied. A delta winglet pair type vortex generator is placed in a hydrodynamically developed and thermally developing laminar channel flow. Computations are done by solving the unsteady, three-dimensional, incompressible Navier-Strokes equations and energy equation using a modified Marker-and-Cell (MAC) method. The flow structure is complex and consists of main, corner and induced vortices. It is observed that as compared to a channel without winglets, the heat transfer is enhanced by 33% when single winglet is used and by 67% when a winglet pair is employed. Effects of thickness of the winglets and Reynolds number on the heat transfer augmentation are presented.

Based on the well-proven 32/ 40 engine, the 32/40 DG dual-fuel engine was developed mainly for stationary applications in cogeneration plants and power stations, covering an output range from 2.4 to 7.2 MW. Forty-one patients were treated with epidural neurolysis using 50 % ethyl alcohol 2 ml. Thirty eight patients were suffering from cancer pain and three patients were complaining of chronic benign pain. Alcohol block was repeated 2.3 times (mean) in the same patient. Thirty patients were followed after the treatment. Forty-seven percent of the patients reported 70 % or greater pain relief and 20 % of the patients reported about 50 % pain relief. Duration of pain relief was from 9 days to 203 days with a mean duration of 54 days. Adverse effects were reported 43 % of the patients. There is no miserable adverse effect. Adverse effect reported most was pain with epidural injection of drugs after the alcohol block had been performed. PMID:8544293

Two experiments used the McGill Pain Questionnaire (MPQ) to examine the affective dimension of pain in patients whose pain was secondary to malignancy. In experiment I, segregating groups of cancer patients on the basis of extreme scores (high versus low) on the MPQ failed to produce segregation on independent measures of affect and infirmity. This outcome contrasts with earlier work with chronic benign pain patients. Experiment II compared cancer pain patients matched with benign pain patients on intensity of pain report on the affective dimension of the MPQ. Cancer pain patients reported a reliably higher affective loading to their pain. The evaluation of hepatic histology and treatment of hepatitis C virus (HCV)/HIV-coinfected patients is rapidly changing. HCV has become an important cause of mortality in HIV-infected patients. Consequently, assessment of liver histology in all coinfected patients is particularly important. The evaluation of hepatic histology is shifting from reliance on the liver biopsy toward noninvasive modalities. Additionally, the importance of HCV-associated morbidity and mortality in HIV-infected patients has prompted increasing numbers of these patients to be HCV treatment candidates. Prospective trials in coinfected patients have reported lower sustained virologic responses compared to HCV-monoinfected patients. Purpose: To evaluate the significance of preoperative localization of abnormal parathyroid glands to the surgical outcome in patients with primary hyperparathyroidism. Material and Methods: Thirty-nine patients with primary hyperparathyroidism were studied preoperatively with US (39 patients), CT (30 patients) and MR imaging (18 patients). The overall diagnostic accuracy for US was 87%, CT 66% and MR 94%. In patients with a single parathyroid adenoma US was the most cost-effective localization technique with a detection rate of 96%. CT had a lower detection rate (78%) but was of particular value for fairly large ectopic adenomas in the root of the neck. MR imaging was a good confirmatory test (93%). Therapeutic effect of p-aminobenzoic acid-N-xyloside Na (K-247) were studied. Eleven patients with a variety of solid tumors were treated with K-247 alone. K-247 was given orally 800 mg daily for 4 weeks. As for side effect of the drug, only mild gastritis was observed in a few patients. Partial response (over 25% reduction of tumor size) with a median duration of two months was observed in 3 patients. These cases were metastatic tumor of lung from the carcinoma of thyroid, metastatic tumors of lung from the carcinoma of kidney, and mediastinal tumor. This report deals with the preliminary results of trials in the DATECA project with stage I, II and III patients with non-seminomatous germ cell tumours. Stage 1 patients were randomized between infradiaphragmatic irradiation and observation. No significant difference in recurrence rates has been observed as yet. Eighteen of 95 patients had recurrence with a median time to relapse of 3 months. Fifteen patients achieved complete remission after treatment by combiantion chemotherapy while 3 patients are still undergoing treatment. Stage II patients received 6 series of cis-platinum, bleomycin, and vinblastine. The patients were initially randomized to receive chemotherapy alone versus chemotherapy plus irradiation. Several types of coatings have previously been visually identified on the surface of 105-K East and 105-K West Basins fuel elements. One type of coating (found only in K West Basin) in particular was found to be a thick translucent material that was often seen to be dislodged from the elements as flakes when the elements were handled during visual examinations (Pitner 1997). Subsequently it was determined (for one element only in a hot cell) that this material, in the dry condition, could easily be removed from the element using a scraping tool. The coating was identified as Al(OH){sub 3} through X-ray diffraction (XRD) analyses and to be approximately 60 {micro}m thick via scanning electron microscopy (SEM). The energy levels of the configuration f{sup 2} in an eight-fold cubic crystal field (CF) have been calculated, and the results are used to explain the experimental spectrum of UO{sub 2}. The fourth-order CF potential turns out to be much smaller than usually assumed for this compound. This has an effect of reducing the J-mixing in the wavefunctions, particularly in the case of the ground state wavefunction. In spite of the strength of the CF, the ground state vertical stroke {sup 3}H{sub 4}T{sub 2} right angle is found to be modified only slightly by the J-mixing effect; it consists of 89.4% {sup 3}He{sub 4}, and the remaining eleven components make up the rest. Very good correlation is obtained between the experimental and simulated energy-level schemes. The predominance of {sup 3}H{sub 4} in the ground state consequently increases the value of the calculated effective magnetic moment. This paper presents the application of hydraulic network analysis to evaluate the performance of butterfly valves in nuclear power plant applications. Required actuation torque for butterfly valves in high-flow applications is often dictated by peak dynamic torque. The peak dynamic torque, which occurs at some intermediate disc position, requires accurate evaluation of valve flow rate and pressure drop throughout the valve stroke. Valve flow rate and pressure drop are significantly affected by the valve flow characteristics and the hydraulic system characteristics, such as pumping capability, piping resistances, single and parallel flow paths, system hydrostatic pressure, and the location of the motor-operated valve (MOV) within the system. In a recent survey of the stellar populations of LINERS and LINER/HII Transition Objects (TOs) we identified a numerous class of nuclei which stand out because of their conspicuous 10^8-9 yr populations. These objects were called ``Young-TOs'', since they all have TO-like emission line ratios. In this paper we investigate the radial variations of spectral properties in Low Luminosity AGN. Our analysis is based on high S/N, 3500-5500 A, long-slit spectra for 47 galaxies. The data probe distances of typically up to 850 pc from the nucleus with a resolution of ~ 100 pc and S/N ~ 30. Stellar population gradients are mapped by the radial profiles of absorption line equivalent widths and colours along the slit. These variations are further analyzed by means of a decomposition of each spectrum in terms of template galaxies representative of very young (<= 10^7 yr), intermediate age (10^8-9 yr) and old (10^10 yr) populations. We present the results of a monitoring campaign aimed at deriving rotation periods for a representative sample of stars in the young (30 Myr) open cluster IC 2602. Rotation periods were derived for 29 of 33 stars monitored. The periods derived range from 0.2d (one of the shortest known rotation periods of any single open cluster star) to about 10d (which is almost twice as long as the longest period previously known for a cluster of this age). We are able to confirm 8 previously known periods and derive 21 new ones, delineating the long period end of the distribution. Despite our sensitivity to longer periods, we do not detect any variables with periods longer than about 10d. The combination of these data with those for IC 2391, an almost identical cluster, leads to the following conclusions: 1) The fast rotators in a 30 Myr cluster are distributed across the entire 0.5 < B-V < 1.6 color range. 2) 6 stars in our sample are slow rotators, with periods longer ...

The effect of irradiation on the tensile properties of oil palm empty fruit bunch (OPEFB) fiber reinforced poly(vinyl chloride)/epoxidized natural rubber (PVC/ENR) blends were studied. The composites were prepared by mixing the fiber and the PVC/ENR blend using HAAKE Rheomixer at 150 deg. C. The composites were then irradiated by using a 3.0 MeV electron beam machine at doses ranging from 0 to 100 kGy in air and room temperature. The tensile strength, Young's modulus, elongation at break and gel fraction of the composites were measured. Comparative studies were also made by using poly(methyl acrylate) grafted OPEFB fiber in the similar blend system. This paper presents diffraction-limited 1-18 micron images of the young quadruple star system HD 98800 obtained with the W. M. Keck 10-m telescopes using speckle and adaptive optics imaging at near-IR wavelengths and direct imaging at mid-IR wavelengths. The two components of the visual binary, A and B, both themselves spectroscopic binaries, were separable at all wavelengths, allowing us to determine their stellar and circumstellar properties. Combining these observations with spectroscopic data from the literature, we derive an age of 10 Myr, masses of 0.93 and 0.64 M_sun and an inclination angle of 58 deg for the spectroscopic components of HD 98800 B, and an age of 10 Myr and a mass of 1.1 M_sun for HD 98800 Aa. Our data confirm that the large mid-IR excess is entirely associated with HD 98800 B. This excess exhibits a black body temperature of 150 K and a strong 10 micron silicate emission feature. Brine from active oil wells seeped through the soil of a forested site in north-western Pennsylvania and killed all vegetation in its path, leaving the affected area unproductive and unsightly. After the brine source was eliminated, herbaceous plants, shrubs and forest tree seedlings became established and developed rapidly. Establishment began in the first year and by Year 4 the site had developed into a healthy young forest. Results show that Allegheny hardwood forest sites damaged by brine water will rapidly revegetate once the brine water discharge is stopped if there is a seed supply and if the area is fenced to exclude deer. 3 figs., 12 refs.

Methods are described for the production of callus and subsequent shoot and root induction and multiplication using stamen filaments, nodes of young stems of mature trees or cotyledon petioles. A high frequency of contamination occurred using nodal sections from mature trees, whereas up to 100% sterility was obtained with cotyledons and stamens. The rooting % of shoot cultures from nodes or stamen callus was low compared with that of shoots from seedling callus. Considerable variation was observed between clones from different trees in their ability to produce shoots and roots. 27 references

The effect of protein malnutrition on the metabolism of collagen in bone was studied in young female albino rats after a single injection of "3H-proline. Both specific and total radioactivities of hydroxyproline in the total collagen of the bone were found to decrease in the protein-deficient animals, indicating decreased rate of collagen synthesis. In the urine the amount of hydroxyproline excreted and total radioactivity of "3H-hydroxyproline were greatly decreased. The results of the present investigation therefore clearly indicate decreased synthesis and catabolism of collagen in bones of protein deficient animals compared to controls. (auth.).

Purpose: Treatment results and the pattern of relapse were evaluated in the multimodal treatment of Ewing's sarcomas of the chest wall. Methods and Materials: In a retrospective analysis, 114 patients with non-metastatic Ewing's sarcoma of the chest wall were evaluated. They were treated in the CESS 81, CESS 86, or EICESS 92 studies between January 1981 and December 1993. The treatment consisted of polychemotherapy (VACA, VAIA, or EVAIA) and local therapy, either surgery alone (14 patients), radiotherapy alone (28 patients) or a combination of both (71 patients). The median follow-up was 46.6 months (range 5-170). A relapse analysis for all patients with local or combined relapses was performed. Thirty-two asthmatic volunteers 8 to 16 yr of age, recruited through local schools and private physicians, were exposed in a chamber to clean air (control condition) and to sulfuric acid aerosol at a low concentration (46 +/- 11 micrograms/m3; mean +/- SD) and at a high concentration (127 +/- 21 micrograms/m3). Acid aerosols had mass median aerodynamic diameters near 0.5 microns with geometric standard deviations near 1.9. Temperature was 21 degrees C, and relative humidity was near 50%. Subjects were exposed with unencumbered oronasal breathing for 30 min at rest plus 10 min at moderate exercise (ventilation rate approximately 20 L/min/m2 of body surface). A subgroup (21 subjects) were exposed similarly to clean air and to high acid (134 +/- 20 micrograms/m3) with 100% oral breathing. Staging bone scans or skeletal surveys were obtained of 97 patients with endometrial carcinoma. Of the 77 patients with Stage I or II disease, no metastases were identified at staging. Three patients in the entire series demonstrated bony metastases; all of these metastases were detectable by radionuclide bone scan and radiographic bone survey. Eighty-nine patients were examined with radionuclide liver/spleen scanning at the time of staging. Four of the 89 initial scans were interpreted as demonstrating hepatocellular disease, and all four patients had abnormal liver function studies. Only one patient demonstrated a possible hepatic metastasis at initial diagnosis. This patient also had abnormal liver function studies. Computerized tomography of the adrenal glands was performed in 10 patients with Cushing's syndrome using a G.E. 8800 CT/T Body Scanner. The tomographic findings of unilateral adrenal masses in 4 patients were confirmed by surgery. In the remaining 6 patients, computerized tomography did not identify any masses, and both glands were well visualized and showed normal shape. In these patients, measurement of the adrenal glands revealed both normal (n=1) and enlarged glands (n=5). The findings of adrenalectomy (n=4) or transphenoidal surgery and follow-up (n=2) established the diagnosis of adrenal phyperplasia in all patients. (Author).

Thirty-nine previously untreated patients with squamous cell carcinoma of the tongue were treated by irradiation or a combination of irradiation and surgery, with or without chemotherapy, between January 1971 and December 1980. All of the patients had a follow-up period of at least nine years. Twenty-two patients were men and 17 were women. The average age was 50.1 years, with a range of 30 to 74 years. The absolute five-year survival rate was 82.1% and the cumulative ten-year survival rate 76.9% for these 39 patients. The prospective study includes 25 patients without pathology of the femoral head for the evaluation of the normal femoral head perfusion. In addition 34 patients with femoral head necrosis underwent i.a. DSA preoperatively before pedicled pelvic bone grafting. 15 patients after pelvic bone graft operation and 7 patients with medial femoral head fracture were also examined via superselective DSA. In cases with femoral head necrosis a rarefaction or interruption of the rami nutricii proximales, or an occlusion of the medial circumflex femoral artery were observed. Patients with medial femoral neck fracture showed an interruption of the rami nutricii proximales of the femoral head. Objective: To compare pediatric patients with periventricular leukomalacia (PVL) with normal children by using diffusion tensor MR imaging. Methods: Diffusion tensor images were obtained in 15 pediatric patients with PVL and 15 age-matched normal children. Regions of interest were drawn to measure the fractional anisotropy (FA) in bilateral posterior limb of internal capsule, bilateral optic radiation, genu of corpus callosum, and splenium of corpus callosum. The values of PVL patients and normal children were compared using non-dependent samples T-test. Results: The FA values of regions of interest prescribed were significantly lower in PVL patients than in normal children (P<0.01). Conclusion: Diffusion tensor imaging may reveal retard of myelination of fiber tracts in PVL patients. It can be a potential tool in evaluating the brain development of children. (authors)

This study is for chemoradiotherapy of unresectable and recurrent cholangiocarcinoma (CCC). Between April 2005 and March 2007, 5 patients were evaluated for unresectable and recurrent CCC. All patients were performed percutaneous transhepatic biliary drainage (PTBD) and treated with chemoradiotherapy. The radiation method was an external beam radiotherapy (EBRT) and remote after loading system (RALS). The chemotherapy was 5-fluorouracil (FU) and gemcitabine by intravenous injection. Three patients had unresectable and 2 had recurrent CCC. As for the side effects, 4 patients had a slight neutropenia and 1 had an uncontrollable gastric bleeding. No patient had cholangitis. All patients with von Willebrand?s disease (vWD) who showed an arterial or venous thrombosis and were reported in the literature have been evaluated. 11 patients had arterial thrombosis while 19 had venous thrombosis for a total of 30 cases. 9 out the 11 cases with arterial thrombosis had myocardial infarction. Two had cerebral thrombosis. Associated risk factors for arterial thrombosis were available only for three patients who showed, respectively, smoking and dyslipidemia (2 cases) and smoking and intravenous desmopressin infusion (1 case). The majority of patients with venous thrombosis showed DVT with or without PE. Four patients presented with apparently isolated PE.
