The utilization of health care services in children and adolescents with chronic benign pain was studied in a Dutch population sample of 254 chronic pain sufferers aged 0-18 years. Children and adolescents who had reported chronic pain (continuous or recurrent pain >3 months) in our previous prevalence study were asked to keep a 3-week diary on their pain and to fill out questionnaires on background factors, health care use and the impact of pain. Parent ratings were used for children aged 0-11 years, self-report was used in adolescents (12-18 years). In a 3-month period, in 53.4% of the cases medication was used for pain, and general practitioners and specialists were consulted for pain in 31.1% and 13.9% of subjects, respectively. Almost 900m adolescents and adults are illiterate in the developing world, yet most policy discussions focus on the educational circumstances of primary aged children. As a result non-formal educational programs for adolescents and adults are given very little support, and this group is virtually ignored in international agreements such as the millennium declaration. This article presents the first serious attempt at evaluating the impact of a non-formal education program. Results show significant learning achievement over the academic year, as well as strong development impacts of the program in non-learning dimensions of participants' lives. We studied age-related microstructural changes in the subcortical white matter from adolescence to middle age in 16 healthy male subjects (age range: 12 to 47 years) using diffusion-weighted magnetic resonance (MR) imaging (DWI). Mean diffusivity (MD) of subcortical white matter was mapped onto the overlying cortical surface using our original method. The mean MD values within 76 cortical regions were measured using atlas-based gray matter segmentation and then correlated with age. A significant negative correlation with age was observed in subcortical MD values in several cortical regions, primarily over the bilateral temporal and occipital lobes. This study explored adolescents' sexual self-disclosure in real life and cyberspace, as well as gender differences of sexual self-disclosure in cyberspace. There were 115 male and 92 female adolescents who participated in this pretest and posttest survey. In general, the depth of sexual self-disclosure in real life was greater than in cyberspace, but the breadth of sexual selfdisclosure in both real life and cyberspace was equivalent. The cross-lagged panel analysis indicated that adolescents who were willing to self-disclose sexual topics in real life also selfdisclosed them more in cyberspace, and vice versa. Furthermore, sexual self-disclosure in both disclosing environments was mutually predicted by each other. Ewing's sarcoma is the most common malignant bone tumour occurring in children and adolescents and exists in two different clinico pathological entities: osseous Ewing's sarcoma (OES) and extraosseous Ewing's sarcoma (EES). Five cases of primary epidural EES are described, which presented with non-specific symptoms leading to a long diagnostic delay. The median age at diagnosis was 22 years (range 13-36 years). The median diagnostic delay was 3 months. All patients had one or more neurological deficits. All underwent surgical exploration with a laminectomy and partial resection followed by adjuvant radiotherapy to a dose of 46-50 Gy and chemotherapy with VAC (vincristine, adriamycin and cyclophosphamide) alternating with ICE (ifosphamide, cisplatin and etoposide) for at least six cycles. Bisphenol-A (BPA) is one of the most common environmental endocrine disrupters and has a wide range of effects on central nervous system. Adolescence is another important developmental period besides the early critical prenatal and neonatal periods. In the present study, we exposed mice to BPA (40, 400 mg/kg/d) between adolescence and young adulthood for 8 weeks and investigated its effects on the non-reproductive behaviors. To determine the demographics, imaging findings, clinical symptoms, and prognosis of primary vertebral Ewing's sarcoma (PVES). A retrospective review of medical records and radiological studies of patients diagnosed with PVES from 1936 through 2001 in our institution and Department of Pathology consultation files was undertaken. Metastatic and soft tissue Ewing's sarcoma cases were excluded. From a total of 1,277 cases of Ewing's sarcoma, 125 (9.8%) had a primary vertebral origin. There were 48 females and 76 males. Patient ages ranged from 4 to 54 (mean 19.3, standard deviation 10.7, median 16) years. Vertebral column distribution was four cervical (3.2%), 13 thoracic (10.5%), 31 lumbar (25%), and 67 sacrum (53.2%). More than one vertebral segment was involved in ten cases (8%). A 36-year-old woman underwent amniocentesis for advanced maternal age. The fetal karyotype had an extra dark staining G band on the long arm of chromosome 11 with no other identifiable abnormalities. FISH studies using a chromosome 11 paint probe confirmed the origin of the extra band. The abnormality was identified as a partial duplication of 11q: 46,XX dir dup (11)(q13.5q21) or (q21q23.1). Three adolescents (ages 14-17) with emotional and behavioral disorders displayed chronic disruptive behavior in their self-contained classrooms at a self-contained alternative school. A descriptive functional behavioral assessment was conducted for each student. Data from file review, structured interviews, and direct observations were used to identify the functions of their disruptive behaviors. Then, function-based interventions were systematically constructed for each student and implemented for an extended period (nearly 6 weeks) within the most problematic situation in their classrooms. The interventions improved each student's behavior and the effects maintained during follow-up and generalized to instruction in a nonintervention classroom. Social validity data comparing the interventions to baseline practices revealed the function-based intervention had moderately higher social validity among teachers and substantially higher social validity among students. ...

This paper reports the findings of a study that explores the perspectives of 25 young Cypriots regarding the symbolic values of smoking with respect to their everyday lived experiences. This study was motivated by recognising the high levels of smoking prevalence among adolescents in Cyprus and the absence of published research 'recording' their own versions of reality about smoking. A convenience sample of 230 youth (mean age = 14.1, standard deviation = 2.2; 24% used sign only, 40% speech only, and 36% sign + speech) was surveyed on communication-related issues, generic and DHH-specific quality of life, and depression symptoms. This introductory paper surveys potential sources of electric magnetic, and electro-magnetic fields. Various cases are discussed to exemplify the total frequency range: nuclear magnetic resonance tomography, high-voltage transmission lines, transformer stations, effect lighting balls, military transmitters, transmitter towers of the Postal Services and other operators, mobile radiotelephone equipment, large broadcasting transmitters, radar radiation, high-frequency heat therapy. There is evidence suggesting that electric, magnetic and electro-magnetic fields may possibly represent a certain nuisance or health hazard even at field strength occuring in equipment used for every-day-life purposes, with an emphasis on their possible actions and effects in children and adolescents. Method:- We compared 378 young people under the age of 18 who received a diagnosis of bipolar disorder and/or ADHD from a large NHS mental health trust between 1992 and 2007. Results:- Children with bipolar disorder were rare in this sample (n-=-35, 1.0%), particularly under the age of 13 (n-=-9, 0.3%). In view of the anticipated increased use of atomic energy in industry, the possible long-term effects of chronic radiation exposure were studied in five generations of rats. Female rats (F_0) were given tritiated drinking water ("3HOH;3#mu#Ci/ml) from adolescence (60 days) until and throughout pregnancy. A separate study showed that the maximum radioactivity in the urine is reached in 30 days, and in the blood in 42 days. In the newborns, the highest specific activity was in the nucleic acid fraction, but total radioactivity was mainly due to the water (body fluids) compartment. No signs of radiation illness or increase in cataract formation in the mothers were observed. The food and water intake and body weight changes before pregnancy were normal. The ultimate compressive strength and modulus of elasticity of femoral cortical bone from adult geese (Anser anser), were determined by sex and by quadrant by compressing small right circular cylinders which were 2.4 mm in height and 0.8 mm in diameter. The average ultimate compressive strength was 183 +/- 29 MPa. The average modulus of elasticity was 13.2 +/- 3.4 GPa. The bending strength and bending modulus of elasticity were determined by a three point bend test on rectangular prisms which had the approximate dimensions 0.75 mm X 0.75 mm X 25 mm. The average bending strength was 263 +/- 44 MPa while the average bending modulus was 19.6 +/- 3.1 GPa. The calcium content was determined by atomic absorption spectrophotometry and no correlation was found with the mechanical properties. Purpose: The results of therapy in 288 men with pathologic Stage C prostate cancer who underwent radical retropubic prostatectomy (RRP) were analyzed to determine the effects of adjuvant therapy. Methods and Materials: Twenty-seven of the 288 patients received preoperative neoadjuvant hormonal therapy (leuprolide acetate). Postoperatively, 60 patients received adjuvant radiotherapy (RT) to the prostate bed. Follow-up ranged from 3 to 83 months (median = 32 months). Freedom from failure (FFF) was defined as maintaining a serum PSA level of #<=# 0.3 ng/ml. Results: The FFF was 61% at 3 years and 45% at 5 years for the entire group. Evidence from twin, adoption, and family studies suggests that there is strong aggregation of suicidal behaviors in some families. By comparison, the role of social modeling through peers has yet to be convincingly established. This paper uses data from four large studies (the WHO/EURO Multicentre Study on Suicidal Behaviour, the WHO/SUPRE-MISS, the CASE study, and the Queensland Suicide Register) to compare the effects of exposure to fatal and nonfatal suicidal behavior in family members and nonfamilial associates on the subsequent suicidal behavior of male and female respondents of different ages. Across all studies, we found that prior suicidal behaviors among respondents' social groups were more important predictors of suicidal behavior in the respondents themselves than previous research had indicated. Community-based suicide attempters in the WHO SUPRE-MISS had higher rates of exposure to prior suicide in nonfamilial associates than in family members. In an ...

Ewing's sarcoma has retained the most unfavorable prognosis of all primary musculoskeletal tumors. Prior to the use of multi-drug chemotherapy, long-term survival was less than 10%. The development of multi-disciplinary therapy with chemotherapy, irradiation, and surgery has increased current long-term survival rates in most clinical centers to greater than 50%. In addition, the preferred method of tumor resection has changed; limb salvage has nearly replaced amputation of the affected limb. Limb salvage procedures can be performed in place of amputation without compromising patient survival rates. Candidate materials for components of a fluidized-bed waste heat recovery system (FBWHRS) include plain carbon steel, types 405 and 316 stainless steel, and alloy 800. These materials were exposed to synthetic flue gas at anticipated FBWHRS temperatures and higher for 3000 h to determine corrosion rates. The synthetic flue gas was a combustion atmosphere to which Cl/sub 2/ and HCl were added. The projected annual corrosion rate for plain carbon steel is about 0.08 mm/year (0.003 in./year) at 250 to 290/sup 0/C, but it would exceed 0.5 mm/year (0.02 in./year) at 560/sup 0/C. The implementation of the experiment is divided in two consecutive phases. The use of Gadolinium-DTPA as a paramagnetic contrast agent in MRI with adults and juveniles concerning brain and spinal cord pathology is well proven since years. In the FRG it is only introduced for children over two years of age. Therefore this report deales with the experience in four infants and small children under the age of two. They all were suffering from neurological tumors and got additional diagnostic information in three cases. Short rotation trials using cuttings poplar (Populus x rasumowskyana) in Southern Finland investigated the establishment of poplar plantations and the effects of spacing and application of nitrogen fertilizer on biomass production over a period of 6 years. Thicker cuttings grew better whilst those of less than 1 cm diameter grew only moderately. Nitrogen fertilization improved height and diameter growth and above-ground dry mass yield. Woody biomass production was 4.2 dry tons/ha per year, at 300 kg/ha nitrogen. A spacing of 15 000 stems/ha gave the best yield after 4 years, but 5000 stems/ha was more productive spacing in the next 2 years. (author).

MRI is a powerful diagnostic modality of expanding availability. Equipment and installation amount to nearly $2M per site. An important component of diagnostic efficacy is not just equipment costs but also replacement costs, which for x-ray CT amount to 14-20% of the purchase cost per year; and in the early years of that technology reached 30-50%. The authors show how design choices made during the RandD stages of MRI development have allowed them to improve system performance parameters such as data reconstruction, archiving and display speeds, computational capabilities, operator interfaces, imaging sequence flexibility and signal-to-noise levels. Over the last four years these improvements have been made at a retrofit cost well below the target of 6-7% of the purchase price per year.

All of the patients had a follow-up period of at least nine years. Twenty-two patients were men and 17 were women. The average age was 50.1 years, with a range of 30 to 74 years. Of 1850 patients with cholelithiasis diagnosed in the past 17 years, 1116 female and 734 male, 598 patients (32.3%) presented with one or more of three major symptoms, i.e., abdominal pain, fever and jaundice, whereas the remainder (67.7%) had none of these symptoms. The proportion of the asymptomatic patients was similar in all age groups, being around 70%. Only 20 per cent of 680 asymptomatic patients, followed for 10 to 17 years (median 13.3 years), developed biliary symptoms. Older patients over 70 years of age had a higher rate of change to the symptomatic group, as compared with younger patients under 70, 29.5% vs. 19.3%, respectively. During this period, carcinoma of the gallbladder developed in one of the asymptomatic patients (0.1%). We examine patterns of variation of 32 primarily main sequence stars, extending our previous 7-12 year time series to 13-20 years by combining b, y data from Lowell Observatory with similar data from Fairborn Observatory. Parallel chromospheric Ca II H and K emission data from the Mount Wilson Observatory span the entire interval. The extended data strengthen the relationship between chromospheric and photometric variation derived previously. Twenty-seven stars are deemed variable. On a year-to-year timescale young active stars become fainter when their Ca II emission increases while older less active stars such as the Sun become brighter when their Ca II emission increases. The grant covered the one-year operational expenses of a worldclass core and cuttings facility located in Houston, Texas, that BP America donated to the BEG. The DOE investment of $300,000, matched by a $75,000 UT contribution, provided critical first-year funds that were heavily leveraged by the BP gift of $7.0 million in facilities and cash. DOE also provided a one-month extension and grant of $30,000 for the month of May 2003. A 5-year plan to grow a permanent endowment in order to manage the facility in perpetuity is well under way and on schedule. During the autumn of 1994 an investigation of foodstuffs has been accomplished to assess the average intake of {sup 137}Cs by the Swedish population due to the Chernobyl accident. A standardized food basket has been collected from two grocers in 10 localities, of which the majority came from areas with the highest fallout. The estimated maximum intake of {sup 137}Cs was 815 Bq/year in the inland of the county of Vaesterbotten. The population weighted average intake for the fallout affected counties was 435 Bq/year. The rest of the county received an intake of 235 Bq/year. The population weighted average of the intake for the whole county was estimated to 274 Bq/year. Lack of access to basic education leads to diminished individual and national capabilities, therewith furthering cycles of poverty. An equitable education system meeting basic learning needs represents not only a human right, but also a means for reducing poverty, promoting productivity, and sustaining development. The Government of China--the most populous developing nation, the majority of whose citizens live in rural areas--has been committed to universalizing nine-year compulsory education among school-aged children and eliminating illiteracy among youths and adults aged 15-45. This study examines lessons learned from China's efforts in these areas. It also reports on current challenges and trends in a new national initiative for achieving high-quality universal basic education by the year 2007.

Twenty-four long-term survivors of Ewing's sarcoma were identified as being at risk for a second primary tumor. Among this group of patients followed from 3 to 22 y, 4 new bone tumors were observed, whereas 1.2 x 10"-"3 were expected. All new tumors arose in heavily irradiated areas. The risk associated with radiation after 3 years was 7.2 cases/million person-years per rad. The cumulative cancer risk over 10 years for irradiated patients was 35% (SE, 15.1%). Intensive chemotherapy (cyclophosphamide and vincristine administered in five or more courses) seemed to exert an enhancing effect, increasing the rate of development of new tumors.

Lifetime observations on a group of 358 rhesus monkeys indicate that life expectancy loss from exposure to protons in the energy range encountered in the Van Allen belts and solar proton events is influenced primarily by the dose rather than the energy of radiation. After 24 years, life expectancy losses from similar surface doses of low-LET (138-2300 MeV) and high-LET (32-55 MeV) protons are not significantly different, but the high-LET protons are associated with more deaths in the early years, while the low-LET protons contribute more to mortality in later years. In males, the most significant cause of life shortening is nonleukemia cancers.

The Department of Energy (DOE) Office of Energy Efficiency and Renewable Energy (EERE) is interested in assessing the potential economic impacts of its portfolio of subprograms on national employment and income. A special purpose version of the IMPLAN input-output model called ImBuild II is used in this study of all 21 Building Technologies Program subprograms in the EERE final FY 2005 budget request to the Office of Management and Budget on February 2, 2004. Energy savings, investments, and impacts on U.S. national employment and earned income are reported by subprogram for selected years to the year 2030. Energy savings and investments from these subprograms have the potential of creating a total of 396,000 jobs and about $5.6 billion in earned income (2003$) by the year 2030.

A two-year study of the effects of pH and associated water chemistry variables on detrital processing in three streams with different bedrock geology in the Monongahela National Forest, West Virginia were investigated. Leaf pack processing rates and macroinvertebrate colonization and microbial biomass (ATP concentration) on the packs in the three stream were compared. Abstract Routinely used prognostic factors fail to predict clinical outcome in a significant proportion of breast cancer patients, implying that they can not detect some important biological characteristics. Chromosomal changes have been described in breast carcinomas for many years but their significance is not clear. We compared chromosomal changes with clinico-pathological characteristics and clinical outcome in 203 breast cancer patients with a follow-up of 9-18 years. Colorectal cancer (CRC) is the most common cause of cancer death among Canadian non-smokers. Each year about 21,500 Canadians are diagnosed with CRC. Ninety percent of cases occur in people aged 50 years or older. An estimated 4,800 Canadian men and 4,100 Canadian women die each year from the disease. About 75% of all new CRC cases arise in people with no known risk factors. Computed tomography colonography (CTC), a cross-sectional imaging examination that permits complete structural examination of the colon, shows promise as a tool for CRC screening. This article reviews the rationale for CRC screening, currently recommended Canadian and American screening guidelines and screening tools, and current evidence supporting CTC as a CRC screening tool and its advantages over conventional screening methods. An estimate for the number of ionizing photons per baryon as a function of redshift is computed based on the plausible extrapolation of the observed galaxy UV luminosity function and the latest results on the properties of the escape fraction of ionizing radiation. It is found that, if the escape fraction for low mass galaxies (Mtot<10^{11}Msun) is assumed to be negligibly small, as indicated by numerical simulations, then there are not enough ionizing photons to reionize the universe by z=6 for the cosmology favored by the WMAP 3rd year results, while the WMAP 1st year cosmology is marginally consistent with the reionization requirement. The escape fraction as a function of galaxy mass would have to be constant to within a factor of two for the whole mass range of galaxies for reionization to be possible within the WMAP 3rd year cosmology.

Safety assessments for clearance of H-beam and valves, main radioactive metal wastes in Kori nuclear power plants, were carried out to derivate a decontamination factor for satisfying the dose limits of clearance level (Maximum individual dose: 10 {mu}Sv/y, collective dose: 1 man{center_dot}Sv/y) in Korea. Maximum individual dose and collective dose were evaluated by internal dose conversion factor which based on the concept of effective dose in ICRP publication 60. The results of maximum individual dose and collective dose is 139 {mu}Sv per year and 0.166 man{center_dot}Sv per year about H-beam, and 158 {mu}Sv per year and 0.468 man{center_dot}Sv per year about valves respectively. Demand decontamination factor satisfied with, which is respectively more than 13.9 and 15.8 for satisfying clearance level.
