The results of radical retropubic prostatectomy and adjuvant therapy for pathologic stage C prostate cancer

Purpose: The results of therapy in 288 men with pathologic Stage C prostate cancer who underwent radical retropubic prostatectomy (RRP) were analyzed to determine the effects of adjuvant therapy. Methods and Materials: Twenty-seven of the 288 patients received preoperative neoadjuvant hormonal therapy (leuprolide acetate). Postoperatively, 60 patients received adjuvant radiotherapy (RT) to the prostate bed. Follow-up ranged from 3 to 83 months (median = 32 months). Freedom from failure (FFF) was defined as maintaining a serum PSA level of #<=# 0.3 ng/ml. Results: The FFF was 61% at 3 years and 45% at 5 years for the entire group. The FFF following RRP plus RT was 75% at 3 years and 57% at 5 years as compared to 56% at 3 years and 40% at 5 years for RRP without RT (p = 0.049). The survival of acetabular components depends on several factors: wear, osteolysis and septic or aseptic loosening. Osteolysis seems to be the main cause for concern in cementless arthroplasties. Acetabular osteolysis results from particle debris and segmental unloading of acetabular bone by rigid sockets. We investigated a cementless elastic monoblock socket with regard to acetabular osteolysis and aseptic loosening in a cohort of young patients. We evaluated 158 hip arthroplasties with a minimum follow-up of ten?years (ten to 18) and a mean age of 42?years (18?50). This paper provides both a detailed history of environmental change in the Sierra Nevada over the past 1,800?years and evidence for climate teleconnections between the Sierra Nevada and Greenland during the late Holocene. A review of Greenland ice core data suggests that the magnitudes of abrupt changes in temperature and precipitation increased beginning c. 3,700 and 3,000?years ago, respectively. Maps of wind resources were used to develop a method of evaluating climatic changes and their potential impacts on wind energy resources. Global IPCC climate change scenarios were used to predict climatic conditions for the future, while past wind resource availability was simulated and validated using NCEP and NCAR reanalysis data as well as observed meteorological data from Environment Canada. The simulations were used to compare each 5-year period with a 50-year reference period. Regional scale climate change impacts were evaluated using a statistical dynamic down-scaling method. Advanced meteorological models were used to predict wind flow patterns across specific landscapes. The evolution of past wind resource availability was then simulated. Corrosion of fluidized-bed boiler materials in synthetic flue gas

Candidate materials for components of a fluidized-bed waste heat recovery system (FBWHRS) include plain carbon steel, types 405 and 316 stainless steel, and alloy 800. These materials were exposed to synthetic flue gas at anticipated FBWHRS temperatures and higher for 3000 h to determine corrosion rates. The synthetic flue gas was a combustion atmosphere to which Cl/sub 2/ and HCl were added. The projected annual corrosion rate for plain carbon steel is about 0.08 mm/year (0.003 in./year) at 250 to 290/sup 0/C, but it would exceed 0.5 mm/year (0.02 in./year) at 560/sup 0/C. The projected annual corrosion rates of the other materials were less than 0.25 mm/year (0.01 in./year) at the maximum test temperatures of 560, 660, and 665/sup 0/C for type 405, type 316, and alloy 800, respectively. Simple correlations and corresponding nomographs are presented, which express the maximum useful heat received from salt-gradient solar ponds throughout the year or during a specified season of the year, and the corresponding optimum depth of the nonconvective zone in terms of the thickness of the upper convective zone and the temperature under which the maximum useful heat is received. The 'elementary' particle physics began in 1935, when Hideki Yukawa published his pioneering pi-meson theory, and the problem of strong interaction was finally solved 40 years later by the establishment of the Standard Model. The composite models of hadrons by the Sakata school and Sin-itiro Tomonaga's renormalization theory for quantum electrodynamics played essential roles for finding this beautiful solution. It is really surprising that it took only 40 years to solve such desperately difficult problem. The use of Gadolinium-DTPA as a paramagnetic contrast agent in MRI with adults and juveniles concerning brain and spinal cord pathology is well proven since years. In the FRG it is only introduced for children over two years of age. Therefore this report deales with the experience in four infants and small children under the age of two. They all were suffering from neurological tumors and got additional diagnostic information in three cases. The fracture healed, and over the next 2.5 years there was further refracturing and healing with progressive osteolysis. A biopsy was performed and revealed Ewing's sarcoma. The disease was locally controlled by neoadjuvant chemotherapy and radiation therapy. The patient is disease free with excellent function 6 years following the discovery of the lesion. For ARM, the metrics will produce and make available new continuous time series of radiative fluxes based on one year of observations from Barrow, Alaska, during the International Polar Year and report on comparisons of observations with baseline simulations of the Community Climate System Model (CCSM).

Short rotation trials using cuttings poplar (Populus x rasumowskyana) in Southern Finland investigated the establishment of poplar plantations and the effects of spacing and application of nitrogen fertilizer on biomass production over a period of 6 years. Thicker cuttings grew better whilst those of less than 1 cm diameter grew only moderately. Nitrogen fertilization improved height and diameter growth and above-ground dry mass yield. Woody biomass production was 4.2 dry tons/ha per year, at 300 kg/ha nitrogen. A spacing of 15 000 stems/ha gave the best yield after 4 years, but 5000 stems/ha was more productive spacing in the next 2 years. (author).

MRI is a powerful diagnostic modality of expanding availability. Equipment and installation amount to nearly $2M per site. An important component of diagnostic efficacy is not just equipment costs but also replacement costs, which for x-ray CT amount to 14-20% of the purchase cost per year; and in the early years of that technology reached 30-50%. The authors show how design choices made during the RandD stages of MRI development have allowed them to improve system performance parameters such as data reconstruction, archiving and display speeds, computational capabilities, operator interfaces, imaging sequence flexibility and signal-to-noise levels. Over the last four years these improvements have been made at a retrofit cost well below the target of 6-7% of the purchase price per year.

Thirty-nine previously untreated patients with squamous cell carcinoma of the tongue were treated by irradiation or a combination of irradiation and surgery, with or without chemotherapy, between January 1971 and December 1980. All of the patients had a follow-up period of at least nine years. Twenty-two patients were men and 17 were women. The average age was 50.1 years, with a range of 30 to 74 years. The absolute five-year survival rate was 82.1% and the cumulative ten-year survival rate 76.9% for these 39 patients. Cervical node metastases were clinically found on admission in 14 patients whose five-year survival rate was 64.3%, whereas it was 92.0% for 25 patients without metastasis. Purpose: During recent years, more intensified systemic and local treatment regimens have increased the 5-year survival figures in localized Ewing's sarcoma to more than 60%. There is, however, concern about the risk of second malignancies (SM) in long-term survivors. We have analyzed the second malignancies in patients treated in the German Ewing's Sarcoma Studies CESS 81 and CESS 86. Materials and Methods: From January 1981 through June 1991, 674 patients were registered in the two sequential multicentric Ewing's sarcoma trials CESS 81 (recruitment period 1981-1985) and CESS 86 (1986-1991). Of 1850 patients with cholelithiasis diagnosed in the past 17 years, 1116 female and 734 male, 598 patients (32.3%) presented with one or more of three major symptoms, i.e., abdominal pain, fever and jaundice, whereas the remainder (67.7%) had none of these symptoms. The proportion of the asymptomatic patients was similar in all age groups, being around 70%. Only 20 per cent of 680 asymptomatic patients, followed for 10 to 17 years (median 13.3 years), developed biliary symptoms. Older patients over 70 years of age had a higher rate of change to the symptomatic group, as compared with younger patients under 70, 29.5% vs. 19.3%, respectively. During this period, carcinoma of the gallbladder developed in one of the asymptomatic patients (0.1%). Patterns of photometric and chromospheric variation among Sun-like stars: A 20-year perspective

We examine patterns of variation of 32 primarily main sequence stars, extending our previous 7-12 year time series to 13-20 years by combining b, y data from Lowell Observatory with similar data from Fairborn Observatory. Parallel chromospheric Ca II H and K emission data from the Mount Wilson Observatory span the entire interval. The extended data strengthen the relationship between chromospheric and photometric variation derived previously. Twenty-seven stars are deemed variable. On a year-to-year timescale young active stars become fainter when their Ca II emission increases while older less active stars such as the Sun become brighter when their Ca II emission increases. The DOE investment of $300,000, matched by a $75,000 UT contribution, provided critical first-year funds that were heavily leveraged by the BP gift of $7.0 million in facilities and cash. DOE also provided a one-month extension and grant of $30,000 for the month of May 2003. The radiographic appearance of Ewing's sarcoma was studied retrospectively in 22 patients who survived 5 years or longer after diagnosis and treatment. Expected changes from treatment, including regression of the extraosseous soft tissue mass, periostitis, and reconstitution of the cortex, occurred in all patients. Local recurrence occurred in one patient 10 years after complete remission whereas secondary osteosarcoma occurred more than 5 years after complete remission in two other cases. A standardized food basket has been collected from two grocers in 10 localities, of which the majority came from areas with the highest fallout. The estimated maximum intake of {sup 137}Cs was 815 Bq/year in the inland of the county of Vaesterbotten. The population weighted average intake for the fallout affected counties was 435 Bq/year. Patients and Methods: Between March 1996 and December 2000, 169 patients were treated radically for muscle-invasive bladder cancer. Data were collected from patient notes. Statistical analyses were performed using Kaplan-Meier methods and Cox proportional hazards regression analysis to compare radiotherapy and surgical outcome data. The Government of China--the most populous developing nation, the majority of whose citizens live in rural areas--has been committed to universalizing nine-year compulsory education among school-aged children and eliminating illiteracy among youths and adults aged 15-45. This study examines lessons learned from China's efforts in these areas. Throwaway versus regeneration of condensate resin: Radwaste impact

The evaluation methodology utilized in deciding to change over Oyster Creek plant operation from chemical regeneration of condensate demineralizer resin beds to throwaway of this spent resin is presented. Both the economic and operational aspects considered are discussed. The evaluation shows that potential savings in the $1.1M range are feasible over a 5 year cycle. Cost sensitivity analysis results define the throwaway versus regeneration breakeven point to be 47 condensate demineralization beds per 5 year cycle. Financial measures taken in order to counteract the effect of the lower oil prices are described. Operating results and technical modifications of the Sasol One, Two, and Three plants are discussed. Sasol believes that crude oil prices will remain volatile for some time and will fluctuate between $10 and $18 per barrel. A steady increase in crude oil prices will then occur in the 1990's. Risk of radiation-related subsequent malignant tumors in survivors of Ewing's sarcoma

Twenty-four long-term survivors of Ewing's sarcoma were identified as being at risk for a second primary tumor. Among this group of patients followed from 3 to 22 y, 4 new bone tumors were observed, whereas 1.2 x 10"-"3 were expected. All new tumors arose in heavily irradiated areas. The risk associated with radiation after 3 years was 7.2 cases/million person-years per rad. The cumulative cancer risk over 10 years for irradiated patients was 35% (SE, 15.1%). Intensive chemotherapy (cyclophosphamide and vincristine administered in five or more courses) seemed to exert an enhancing effect, increasing the rate of development of new tumors.

The calculated radioactive waste inventories of the Turkey Point pressurized water fission reactor (PWR) and the Starfire conceptual fusion tokamak are compared as a function of time from initial start-up to 10,000 years after decommissioning. Only material out of reactor at least one year is considered. The total activity in Ci/W(th) of the Starfire tokamak is slightly greater than that of the PWR during the active lifetimes of the two reactors and beyond 1000 years. However, using reduced activation materials in Starfire can result in about 1/2000 as much long-lived radioactivity as in the fission reactor. It is stressed that comparison of wastes on this basis is not straightforward, since the radioisotopes and methods required for their disposal are different for fusion and fission reactors. 2 refs., 1 fig., 2 tabs.

Lifetime observations on a group of 358 rhesus monkeys indicate that life expectancy loss from exposure to protons in the energy range encountered in the Van Allen belts and solar proton events is influenced primarily by the dose rather than the energy of radiation. After 24 years, life expectancy losses from similar surface doses of low-LET (138-2300 MeV) and high-LET (32-55 MeV) protons are not significantly different, but the high-LET protons are associated with more deaths in the early years, while the low-LET protons contribute more to mortality in later years. Ewing's Sarcoma : a study of 38 cases

A retrospective study of Ewing's Sarcoma at The Armed Forces Malignant Diseases Treatment Centre, Command Hospital, Pune, is presented. A total of 38 cases over a period of thirteen years have been studied. Males predominate slightly, and the common age group on presentation has been in the second and third decade. Humerus, pelvis and femur are the bones commonly affected. The initial line of management has been primarily radiotherapy, which has been over the years supplemented by combination chemotherapy. All the cases achieved remission, though only 32 cases (84 per cent) had complete remission, and the remaining only partial remission. A new method for determining the year of birth, the year of death, and hence, the age at death, of post-bomb and recently deceased elephants has been developed. The technique is based on Accelerator Mass Spectrometry radiocarbon analyses of small-sized samples extracted from along the length of a ge-line of an elephant tusk. The measured radiocarbon concentrations in the samples from a tusk can be compared to the {sup 14}C atmospheric bomb-pulse curve to derive the growth years of the initial and final samples from the tusk. Initial data from the application of this method to two tusks will be presented. Potentially, the method may play a significant role in wildlife management practices of African national parks. Additionally, the method may contribute to the underpinnings of efforts to define new international trade regulations, which could, in effect, decrease poaching and the killing of very young animals.

Are There Enough Ionizing Photons to Reionize the Universe by z=6?

An estimate for the number of ionizing photons per baryon as a function of redshift is computed based on the plausible extrapolation of the observed galaxy UV luminosity function and the latest results on the properties of the escape fraction of ionizing radiation. It is found that, if the escape fraction for low mass galaxies (Mtot<10^{11}Msun) is assumed to be negligibly small, as indicated by numerical simulations, then there are not enough ionizing photons to reionize the universe by z=6 for the cosmology favored by the WMAP 3rd year results, while the WMAP 1st year cosmology is marginally consistent with the reionization requirement. The escape fraction as a function of galaxy mass would have to be constant to within a factor of two for the whole mass range of galaxies for reionization to be possible within the WMAP 3rd year cosmology.

Quantitative bone scintigraphy in prostatic carcinoma - long-term response to treatment

Quantitative bone scintigraphy was performed in 24 patients with prostatic carcinoma before orchiectomy and up to one to four years after operation. The gamma camera count rate was recorded over the lower thoracic and all lumbar vertebrae 4 h after injection of [sup 99m]Tc-MDP. Twelve patients has normal bone scintigrams throughout the study. They showed from two years after operation a slight increase in count rate values compared with the preoperative values, probability due to hormonal changes after orchiectomy and to age-related alterations in skeletal metabolism. Twelve patients had abnormal bone scintigrams. They showed as a response to treatment the flare phenomenon with an increase in count rate over the abnormal vertebrae when measured two weeks after operation followed by a decrease after two months. The gamma camera count rate was recorded over the lower thoracic and all lumbar vertebrae 4 h after injection of "9"9"mTc-MDP. Twelve patients has normal bone scintigrams throughout the study. They showed from two years after operation a slight increase in count rate values compared with the preoperative values, probability due to hormonal changes after orchiectomy and to age-related alterations in skeletal metabolism. Twelve patients had abnormal bone scintigrams. They showed as a response to treatment the flare phenomenon with an increase in count rate over the abnormal vertebrae when measured two weeks after operation followed by a decrease after two months. Mountain scale modeling of transient, coupled gas flow, heat transfer and carbon-14 migration

We simulate mountain-scale coupled heat transfer and gas flow at Yucca Mountain. A coupled rock-gas flow and heat transfer model, TGIF2, is used to simulate mountain-scale two-dimensional transient heat transfer and gas flow. The model is first verified against an analytical solution for the problem of an infinite horizontal layer of fluid heated from below. Our numerical results match very well with the analytical solution. Then, we obtain transient temperature and gas flow distributions inside the mountain. These distributions are used by a transient semianalytical particle tracker to obtain carbon-14 travel times for particles starting at different locations within the repository. Assuming that the repository is filled with 30-year-old waste at an initial areal power density of 57 kw/acre, we find that repository temperatures remain above 60 degrees C for more than 10,000 years. Forty cases of Ewing's sarcoma of various sites had attended the Department of Radiotherapy at the Wanless Hospital and the Miraj Medical Centre, attached to the Government Medical College, Miraj, Maharashtra, from January, 1970 through December, 1979. There were 28 male patients and 12 female patients in the series with a male to female ratio of 2.3:1. The youngest patient in the series was a female of 6 years of age with the tumour in the scapula and the oldest patient was a 32-year-old male with a lesion in the metacarpal bone. Sixty per cent of the cases were seen in the second decade. The average age of the patients was 13.5 years. The bones of the lower extremities were most commonly involved with the highest number (35 per cent) of the cases occurring in the femur. In some years, this warm current extends much more southward, down to 12/sup 0/S. This has a catastrophic effect leading to a mass mortality of the anchovies and, in turn, of the guano birds and less production of farm crops. From 1964 to 1973 at the Institute of Oncology and Radiobiology, Havana City, 18 clinical histories were studied, all of them from patients with Erwing's sarcoma, histologically diagnosticated by biopsy. Age, sex, race, traumatism background, hemogram disturbances, radiographic signs, primary tumor localization, and early treatment, were exposed. Early treatment in 14(83%) patients as radiotherapy. Local recurrence or metastasis was presented in 10(55,5%) patients during the two first years following attack treatment. Five (35,7%) patients was the survival rate within five years starting early treatemt. After five years, three (21,4%) patients were alive: for 18 and 14 year, and 64 month period; two patients died at 62 and 64 months. The hardware is being supplied by Allied Signal Federal Manufacturing and Technologies, and the packaging will occur at Mason and Hangar Corporation`s Pantex Plant. This case report makes evident that in a 75 years old patient with a (cured) femoral neck fracture 15 years ago a refracture at the same site following a new trauma could not be detected by conventional X-ray. The follow-up of a radionuclide bone study, however, enabled the dignosis of a refracture. From these findings, it is further evident that the decreased bone metabolism in older patients has to be considered. Thus, although the initial bone scan following the trauma is negative, a follow-up scan should be performed within a 4 day-period.

The Federal Government states that an extensive air pollution abatement program, i.e. the revision of existing regulations and implementation of new ones has been allowing the realization of all essential environmental aims originally headed for three years ago in the field of air pollution abatement. Achievements such as these will bring about another obvious improvement of the air quality of the Federal Republic of Germany in the coming years. The recent development of energy economy has also proved to be having an obivously positive effect on the environment and pollution abatement. (HSCH).
