We report a clinical case of severe hepatic fibrosis in an adolescent with severe and recent obesity. Because of elevated serum aminotransferase levels, abnormal hepatic ultrasonography and insulin resistance (impaired glucose tolerance), we suspected nonalcoholic steatohepatitis (NASH). Disease activity and fibrosis were confirmed on liver biopsy. Unilateral diaphragmatic paralysis was diagnosed in 2 preterm neonates born at 29 and 25 weeks of gestation, respectively. In both instances, the pathophysiology was phrenic nerve injury after extravasation of parenteral nutrition fluid. Misplacement and infection were predisposing factors. Diaphragmatic movement analysis by time-motion-mode ultrasonography was helpful in achieving a diagnosis. The first neonate required a diaphragmatic placation, whereas the other infant was managed nonoperatively. These cases confirm a rare etiology of diaphragmatic paralysis and possible spontaneous recovery. Background: the fact that therapeutic irradiation can induce significant stenosis in the arteries of the head, neck, and chest, as welt as in the aorta and the iliac arteries, is familiar in daily practice and well documented in the literature. Patients with heterotaxy syndrome often have complex cardiac and extracardiac anomalies requiring further detailed diagnostic evaluation. They often present severe cardiac failure early in life. Newer radiological modalities in the form of spiral computed tomography (CT) and three-dimensional reconstruction of spiral CT allow clear definition of the anatomy of these anomalies. A 59-year-old woman was diagnosed with polysplenia and multiple anomalies in an abdominal ultrasonography (US) during a control medical examination due to a trivial dietary mistake. Implementing network security guidelines in health care information systems.

The OSHA's chemical occupational exposure standard for laboratories is an outgrowth of the previously issued Hazard Communication Standard. The standard relieves laboratories from complying with general industry standards but does not require compliance with specific guidelines. The heart of the standard is the creation of a Chemical Hygiene Plan (CHP) in every laboratory. The CHP addresses major issues such as safety equipment and procedures, work practices and training, the designation of a chemical hygiene officer, and the provision of medical consultation and examination for affected employees. This new standard, in effect as of 31 January, 1991, presents yet another regulatory challenge to laboratory managers but also will ensure a safer work environment for laboratory workers.

Despite its great usefulness in the performance of the recording procedures prescribed by the X-Ray Ordinance the dose-area product is much less frequently measured for X-ray exposures than during fluorscopic examinations. The great multitude of measuring units, mainly on account of different scales of the measuring equipments themselves, is a cause of uncertainty in clinical routine and should be eliminated as far as the circumstances would permit. As the determination of the organ dose is relevant only in very few cases and does not necessarily require previous measurements of the dose-area product, these are of limited practical value in the assessment of individual radiation exposures during radiographic examinations. Image diagnosis of parathyroid glands in chronic renal failure

Comparative analysis of diagnostic accuracy (dignity determination) in CT-guided fine-needle biopsy (141 patients), CT (141), ultrasonography (89), angiography (50), and cholegraphy (ERCP, 55), was carried out in suspected space-occupying lesions of the pancreas. With accuracy of 86%, sensitivity of 37% and specificity of 92% fine-needle biopsy was superior to imaging procedures. Additional comparison of the results concerning the patients and Roc curves confirm the statements. For reduction of the number of imaging investigations a diagnostic algorithm is proposed, that starts with diagnostic CT and subsequent fine-needle biopsy controlled by CT. This would result in a more adequate use of angiography and ERCP and in the elimination of ultrasonography. (author).

Quality control in medical radiology using T.L.D

Twenty-two out of 31 patients with chronic renal failure and secondary hyperparathyroidism who underwent parathyroidectomy before operation underwent non-invasive image diagnosis of parathyroid glands by computed tomography (CT), scintigraphy with /sup 201/TlCl and /sup 99m/TcO/sup 4 +/, and/or ultrasonography. CT visualized 39 of 45 parathyroid glands (86.7%), weighing more than 500 mg. Scintigraphy with a subtraction method using a computer performed the diagnosis in 19 of 27 glands (70.4%). Ultrasonography detected 21 of 27 glands (77.8%). Image diagnosis was also useful in the postoperative follow-up study. The non-invasive image diagnosis of parathyroid glands in patients with chronic renal failure is thus valuable for 1) definite diagnosis of secondary hyperparathyroidism, 2) localization, and 3) diagnosis for effectiveness of conservative treatment.

In this work tests of quality control in beams of X rays have been carried out, at diagnostic level, of two radiological services equipments of Brazilian hospitals (Aracaju, SE). We obtained the air kerma and kerma rate values, the beams efficiency, the coherence between the luminous fields and the referring fields of irradiation and half-value layer to each equipment and set of parameters. The measures of direct form have been obtained using two ionization chambers, one cylindrical and another one with parallel plates. The value of biliary scintigraphy was studied in 180 patients with suspected biliary tract disease. Most of the patients were investigated additionally by conventional techniques such as cholecystography, cholangiography and ultrasonography. It is concluded that biliary scintigraphy is a simple and safe technique for visualization of the biliary tract. It is particularly useful in the evaluation of acute cholecystitis, in patients with iodine sensitivity obstructive from nonobstructive jaundice.

It needs accurate estimation of radiation level for verifying machinery and cable in Nuclear Power Plant. Therefore, in this study, we used ESR(Electron Spin Resonance) system for estimate dose of Alanine dosimeter. Alanine/ESR dosimetry, already known as a dosimetric method in medical and industrial field, was applied to estimate dose quantity at cable locations within a nuclear power plant as a part of equipment qualification program. Alanine/ESR dosimetry of absorbed dose range is 1 - 100 KGy. The alanine dosimeter is not significantly affected by temperature and fading is limited to 1% per year. A collaboration initiated in 1989 between Judy Lachvayder and Chris Cullis in response to an application for a Christa McCauliffe Fellowship has grown into a substantial program. On the one hand this is due to their longer life expectancy and on the other hand it is due to their higher cell proliferation rate. The paediatric radiology unit of the Inselspital Bern recently installed some of the most advanced X ray equipment nowadays available. It is based on the two latest digital technologies: double read computed radiography (CR) and direct digital radiography (DR). Only the implementation of these digital radiography systems permits the digital acquisition and additionally the analysis of acquired data. The systematic analysis of large amounts of biometric data and exposition data is the basis for further dose reduction and a systematic quality control (QC). Modern imaging methods for diagnostic evaluation of tumors of the oropharynx and the oral cavity

Complex abdominal lesions include a variety of pathologies, such as septated, infected, and hemorrhagic cysts, abscesses, tumors, and fluid collections of different ethiology. These lesions present diagnostic difficulties with both Ultrasonography (US) and Computed Tomography (CT), since findings may not be present or, when present, are not specific. Keeping these limitations in mind, we evaluated 105 patients (111 abdominal lesions) with both US and CT in order to compare their adequacy in predicting the nature of the lesion. On the basis of US and CT results, complex abdominal lesions were divided in four classes: class I includes 43 cases in which both examinations gave the same contribution to the definition of the nature of the lesion, class II (14 lesions, mainly sperimentated cysts), in which US was superior to CT, class III includes 45 cases of hemorrhagic cysts, abscesses, fluid collections and, less frequently, cystic tumors; class IV includes 9 cases in ...

MRI with pathogenic correlation of small renal cell carcinoma

Differentiating a benign from a malignant lesion by information such as density data, or signal intensities or echo gradients, is not possible with CT, nor MRI, nor ultrasonography. Only some lesions show characteristic signs. Inflammations cannot be difscriminated from malignant tumors. CTs of small tumors in particular are so poor in contrast that it is much more difficult to detect tumors or discriminate them from other lesions, as compared to MR images or ultrasonographs. CT does not detect surface tumors, but is more sensitive than MRI or US in detecting discrete cortical bone lesions. The same applies to small calcified tumors. MRI however is most sensitive in detecting tumor volumes in the yellow marrow, and is the best modality for evaluation of extraossal extension of tumors. A delayed case of renal artery pseudoaneurysm presented with gross hematuria and azotemia in solitary kidney following percutaneous nephrostomy: treated by transcatheter coil embolization.

MRI, ultrasonography and angiography were performed on 24 cases with small renal cell carcinoma, and data were compared with pathological architecture type. An average diameter of cancer was 20.8 mm (10-30 mm). Each tumor was pathologically classified into four architecture types: alveolar type (15 cases), papillary type (5 cases), tubular type (3 cases) and cystic type (1 case). In comparison with renal cortex alveolar type renal cell carcinoma showed equal or low signal in T1-weighted images and equal or high signal in T2-weighted images. Papillary and tubular types showed high signal in T1-weighted images, and low signal in T2-weighted images. In alveolar type, many cases showed equal echo in ultrasonography and hypervascularity in angiography, and in papillary and tubular types, many cases showed high echo and hypovascularity. Renal arteriovenous fistula or pseudoaneurysms developing after invasive procedures are either asymptomatic or show only transient symptoms. We describe here a case of symptomatic renal artery pseudoaneurysm that presented with gross hematuria and azotemia. The pseudoaneurysm of renal artery was diagnosed by ultrasonography and abdominal magnetic resonance imaging (MRI), and it was successfully treated by percutaneous transcatheter coil embolization. A high index of suspicion is essential for the early diagnosis and treatment of delayed gross hematuria in patients with history of percutaneous nephrostomy. PMID:17180737

Hepatocellular carcinoma (HCC) is characterized by tumor vascularization from the hepatic artery. The objective of our work was to compare color Doppler ultrasonography (CDU), including power Doppler ultrasonography (PDU) with radiographic imagings with contrast medium in regard to the detection of the arterial tumor vascularity of small hepatocellular carcinomas (HCC). We examined 42 small HCC lesions 2 cm or less in diameter in 37 patients for arterial tumor vascularity by conventional CDU, PDU, dynamic computed tomography (dCT), digital subtraction angiography (DSA), and CT hepatic arteriography (CTA). Color images were detected in 25 (59.5%) and 28 (66.7%) of the 42 lesions with conventional CDU and PDU, respectively, and tumor vascularity was detected in 26 (61.9%) by dCT, 23 (54.8%) by DSA, and 29 (69.0%) by CTA. In the early period after the Chernobyl accident, analysis of patterns of "1"3"1I exposure of the human thyroid showed that contaminated milk was the basic source of "1"3"1I intake among the inhabitants of Russia. The equipment and techniques used for measurement of the "1"3"1I content in the thyroids of these individuals are described in this work. A model of the "1"3"1I intake, taking into account protective actions, and a method of thyroid dose calculation are discussed. The mean thyroid dose and frequency distributions of the thyroid doses to inhabitants of towns and villages of the Bryansk, Tula and Orel regions of Russia are presented. The mean dose to the thyroids of children living in the villages was 2 to 5 times higher than the dose to adult thyroids; for children living in the towns, the mean dose was 1.5 to 12 times higher. Medical explorations by radioisotopes in Lebanon

Only a small number of radiation accidents in the United States have been severe enough to result in multi-organ failure (MOF). Medical details of selected medical misadministration and criticality cases are reviewed, with an emphasis on pathophysiology. The four criticality cases are particularly relevant for analysis of MOF, since medical treatment was supportive and did not appreciably alter the clinical evolution of radiation injury. Sialography, echography and computed tomography in parotid gland study

Retroperitoneoscopic partial adrenalectomy for small adrenal tumours (<=1 cm): the Ruijin clinical experience in 88 patients

The diagnostic accuracy of sialography and ultrasonography (US) in the evaluation of parotid masses is evaluated. Furthermore the role of computed tomography (CT) in this pathology is discussed. In the personal experience US proved to be the best method in the recognition of a parotid tumor while sialography was superior in defining the intra or extraparotid site. The two investigations showed the same accuracy in the definition of benign or malignant nature of the mass. Therefore we consider US the only investigation in most istances; sialography could be performed when the site of the lesion is uncertain or an inflammatory lesion is suspected. CT is never the first investigation; its use is limited to a low number of cases, mainly for the evaluation of large masses and when the association US-sialography does not allow a sure diagnosis.

Summary Penile injury is common as an emergency and should be accurately diagnosed and treated. We analysed 22 patients with penile injury admitted to the emergency unit of Dubrava University Hospital during a 4-year period. According to the American Association for the Surgery of Trauma five-grade classification of penile injuries, there were 14 grade I, 6 grade II and 2 grade III cases. Comparative adequacy of conventional radiography, ultrasonography and Computed Tomography in twenty-six consecutive patients with acute superior mesenteric ischemia

We wanted to assess the usefulness of four-dimensional (4D) ultrasonography (US), i.e., real-time three-dimensional US, as an adjunct for performing various US-guided interventional procedures in superficial lesion. Thirty-three patients were referred for US-guided interventional procedures for superficial lesions, including core biopsy in 19, fine- needle aspriation in eight, therapeutic drug injection in four and needle puncture in two. The procedures were performed under 4D US guidance. We reviewed the pathologic/cytologic results of the core biopsies or needle aspirations, and also the outcomes of drug injection or needle puncture. For all the patients who underwent 4D US-guided core biopsy, the specimens were adequate for making the pathological diagnosis, and specimens were successfully obtained for those patients who underwent 4D US-guided aspiration. Vibration tests on nuclear power station stacks equipped with structural control oil dampers

Ischemic bowel disease is a rare disorder whose incidence is increasing as the mean age of the population increases. Diagnosis by clinical, laboratory and radiologic means is often difficult, and delay in definitive therapy results in substantial morbidity and mortality. A series of 26 consecutive patients, with proved acute superior mesenteric ischemia, was retrospectively reviewed: the authors reports the diagnostic methods performed preoperatively, the site and the cause of infarction and the time passed between the first radiograph and surgery. Plain abdominal radiographs were performed in 25 of 26 patients, screening abdominal US in 23 cases and CT in 19 cases. A Lawrence Livermore National Laboratory (LLNL) Management Pre-Start Review (MPR) Team was formed to independently verify the operational readiness of Building 368 (B368) Biosafety Level III (BSL-3) Facility to conduct research with biological pathogens and toxins including those considered Select Agents. Review objectives and criteria were developed from the DOE/NNSA and LLNL requirements. These were provided in the Implementation Plan for the Biosafety Level III (BSL-3) Facility Management Pre-Start Review (BSL-3 MPR) at Lawrence Livermore National Laboratory that was reviewed and approved by DOE/NNSA-LSO. The formal part of the LLNL MPR for the BSL-3 Facility was begun in August of 2005 but work on the MPR was stopped in October of 2005 due to the need for LLNL to reassess organizational and operational controls and respond to Centers for Disease Control and Prevention inquiries related to a shipping incident involving select agents. In teleradiology, transmitted as well as stored patient data have to be signed digitally according to the currently valid regulation (Roentgenverordnung, RoeV). The epidemiology of gallstone disease in Rome, Italy. Part I. Prevalence data in men. The Rome Group for Epidemiology and Prevention of Cholelithiasis (GREPCO).

Purpose: To evaluate the significance of preoperative localization of abnormal parathyroid glands to the surgical outcome in patients with primary hyperparathyroidism. Material and Methods: Thirty-nine patients with primary hyperparathyroidism were studied preoperatively with US (39 patients), CT (30 patients) and MR imaging (18 patients). The overall diagnostic accuracy for US was 87%, CT 66% and MR 94%. In patients with a single parathyroid adenoma US was the most cost-effective localization technique with a detection rate of 96%. CT had a lower detection rate (78%) but was of particular value for fairly large ectopic adenomas in the root of the neck. MR imaging was a good confirmatory test (93%). In patients with multiple gland disease (primary hyperplasia and multiple adenomas), no single localization study alone was sufficient. Radionuclear estimation with {sup 99m}Tc-N-pyridoxyl-5-methyl-tryptophan (PMT) scintigraphy of recurrent hepatocellular carcinoma after non-surgical treatment

A population of male civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease. Field activities started in December, 1982 and were concluded in July, 1984. Diagnosis was made using real-time ultrasonography. Participation in the study was 71.5%. Prevalence of gallstone disease was 8.2% and increased with age from 2.3% in the 20- to 25-year-old age group to 14.4% in the 60- to 69-year-old age group, based on both presence of gallstones and history of cholecystectomy. About one-third of the subjects with gallstone disease had previously been submitted to cholecystectomy. Only 7.7% of the subjects with presence of gallstones complained of at least one episode of biliary pain in the preceding 5 years. Frequency of "minor" dyspeptic symptoms was not different between men with and those without gallstones. PMID:3391519

Prevalence of hepatosplenomegaly in beta thalassemia minor subjects in Iran

{sup 99m}Tc-N-pyridoxyl-5-methyl-tryptophan (PMT) scintigraphy, which exhibits highly specific for the diagnosis of primary hepatocellular carcinoma (HCC), was performed for recurrent HCC following non-surgical treatment. Eighty-four patients (total 351 examinations) were selected for the study. The ability of PMT scintigraphy to diagnose recurrent HCC after treatment was compared with that of computed tomography, ultrasonography and angiography. The results showed that sixty-three (75%) of 84 cases demonstrated positive findings during our follow-up period, most of the tumors initially showing positive findings remained PMT positive when they recurred and PMT studies shortly after treatment demonstrated wider tracer uptake rather than the actual tumor area. In conclusion, PMT scintigraphy for patients with HCC after non-surgical treatment exhibited high accuracy in identifying viable HCC only for tumors indicating positive findings. (author)

Introduction: Thalassemia is the most common hereditary blood disorder in the world. Iran is located on the thalassemic belt and there is a high prevalence of the hepatosplenomegaly in beta thalassemia minor patients which is reported to be very variable. The goal of this research was to study the frequency of these signs in the cases with beta thalassemia minor patients in Iran. Materials and methods: Two hundred and fifty-nine cases that referred to center for pre-marriage tests were divided into two groups according to their MCV, MCH, and HbA2 (beta thalassemia minor cases and control groups). Liver and spleen sizes were determined by ultrasonographic method and the two groups were compared with each other. Results: Average spleen volumes in case and control groups were 163.48 {+-} 133.97 and 126.29 {+-} 53.98 mm{sup 3}, respectively. A population of female civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease and to define the associated factors. Field activities started in February 1981 and concluded in April 1982. Diagnosis was assessed by real time ultrasonography. Hepatobiliary scintigraphy in current pediatric practice

Ectopically located parathyroid adenomas may be difficult to find during initial neck exploration. They account for over 70 percent of missed adenomas found at reexploration. Preoperative localization of parathyroid adenomas would reduce unnecessary dissection and possibly reduce the number of negative results of initial neck exploration. Before reoperative parathyroid surgery is performed, some means of localization is mandatory to detect ectopic adenomas in the neck and mediastinum. Computed tomography and ultrasonography cannot effectively evaluate the mediastinum. Angiography and venous parathyroid hormone sampling are invasive, costly, and tedious to perform. We have shown that thallium-201 will accurately localize ectopic parathyroid adenomas. As early as 1973, reports published in the radiological literature indicated that this new noninvasive imaging method was a highly effective way of demonstrating intraorbital mass lesions. As CT scanners became widely available, computed tomography became a significant adjunct to ophthalmological diagnosis. The fundamental recommendations of the ICRP issued in its publication of 1990 stipulate the general principle of performance of dose measurements at regular intervals in the field of diagnostic radiology, for the purpose of equipment performance checking and optimization of radiological protection. This paper describes new test methodologies to obtain data used to evaluate the ultimate seismic strength of Nuclear Power Plant (NPP) equipment related to nuclear safety. The paper first reviews existing equipment seismic test data from the viewpoint of the evaluation of their ultimate seismic strength and/or seismic design margins, and extracts the issues in the existing data with regard to their applicability to the evaluation. Then, the paper proposes new test methodologies for the equipment to evaluate their ultimate seismic strength. The test methodology of the equipment employs a vibration amplification system to a shaking table and enhances its applicable maximum acceleration up to 6g. The test methodology herein is cost effective for obtaining test data that is indispensable for evaluating proper seismic margins of NPPs. (author)

Current issues of nuclear equipment standardization in Romania

This paper evaluates activities from 1981 to 1985 of the Donetskgormash association which develops and manufactures equipment for coal mining. The association has manufactured 1,410 hoists, 274 blowers and 26 bucket wheel excavators. Sixteen experimental units of new mining equipment have been developed, among others the RPM 3600-42.5/90 bucket wheel excavator (with a capacity of 3,600 m/sup 3//h for coal handling in storage yards of large power plants), the bucket wheel system used in the EhR 630-10.5/0.5 excavator and the PZ 1600-17/20 materials handling system. Among 20 equipment types manufactured from 1981 to 1985, 4 types were manufactured in the USSR for the first time (e.g. the MPP shaft sinking system and the VOD-50 mine blower). Organ doses received by atomic bomb survivors during radiological examinations at the Radiation Effects Research Foundation

The first part of the study contains a literature compilation of more than 50 original publications reporting the radiation induced effects in 17 different high polymer materials and glass which are relevant in the manufacturing of medical devices or packing materials. The results collected demonstrate that high energy radiation, i.e. gamma- or X-rays, causes various physical and chemical alterations in high polymer materials. A detailed summary and discussion of the results of the original publications is not included in the present report, it will be presented in the second part of the study. Furthermore, the second part of the study will refer to the aspect of wholesomeness of irradiated medical devices or packing materials in a more general manner of representation. (orig.).

Attitudes of Turkish parents, pediatric residents, and medical students toward child disciplinary practices

One of the goals of the DARPA Virtual Soldier Project is to aid the field medic in the triage of a casualty. In Phase I, we are currently collecting 12 baseline experimental physiological variables and a cardiac gated Computed Tomography (CT) imagery for use in an prototyping a futuristic electronic medical record, the "Holomer". We are using physiological models and Kalman filtering to aid in diagnosis and predict outcomes in relation to cardiac injury. The physiological modeling introduces another few hundred variables. Palliation of radiation-related mucositis

Depending on their unique physical properties, proton and heavy ions have taken an irreplaceable role in modern means of tumor treatment. One-step process and two-step process physical models were employed to explain the mechanism of ion energy loss. The transport process of proton and "1"2C in water was simulated by Geant4 toolkit to study the physical properties of ion beam. The calculation results were discussed, which showed the advantages and disadvantages of proton and "1"2C in the medical application. (authors)

Guidelines on the medical therapy of persons accidentally overexposed to ionizing radiations. External contamination

Oral mucositis associated with head and neck radiotherapy can substantially hinder completion of cancer therapy. Alleviation of this often severe stomatitis can provide enhanced patient comfort and facilitate appropriate care. A double-blind format was used in a pilot project to measure, against a control rinse, the effectiveness of an oral rinse consisting of hydrocortisone, nystatin, tetracycline, and diphenhydramine in controlling radiation-related mucositis. A combination of clinical evaluation and patient responses to a questionnaire was used to judge the results of the topical medications. Patients using the experimental medication developed less mucositis than did patients in the control group.

The distribution of measures and responsibilities for the therapy of the irradiated patients with radioactive materials are enumerated. A compact medical synchrotron is being developed which aims to promote the advanced radiotherapy based on the accelerator. As it is small-sized, a high field dipole magnet of 3 T at 200 kA is used. It is excited by the discharge current of the capacitor bank of which waveform is half sinusoidal. To provide with the flat field of 10 - 20 #mu#s duration for the multi-turn injection an additional power supply is developed. For the quadrupole magnet a 100 kHz switch-mode power supply is adopted as it requires the fine tuning and accurate field tracking to the dipole field. (author)

Differentiating a benign from a malignant lesion by information such as density data, or signal intensities or echo gradients, is not possible with CT, nor MRI, nor ultrasonography. Only some lesions show characteristic signs. Inflammations cannot be difscriminated from malignant tumors. CTs of small tumors in particular are so poor in contrast that it is much more difficult to detect tumors or discriminate them from other lesions, as compared to MR images or ultrasonographs. CT does not detect surface tumors, but is more sensitive than MRI or US in detecting discrete cortical bone lesions. The same applies to small calcified tumors. MRI however is most sensitive in detecting tumor volumes in the yellow marrow, and is the best modality for evaluation of extraossal extension of tumors. The polar crane, D-rings, missile shields, refueling canals, refueling bridges, equipment, and elevations 305' and 347'-6'' were flushed with low pressure water. Additionally, floor surfaces on elevation 305' and floor surfaces and major pieces of equipment on elevation 347'-6'' were sprayed with high pressure water. The Lawrence Livermore National Laboratory (LLNL) has the necessary equipment and trained personnel to respond to a large number of hazardous material spills and fires or other emergencies resulting from these spills including injured personnel. This response capability is further expanded by the agreements that LLNL has with a number of outside response agencies. The Hazards Control Department at LLNL functions as the central point for coordinating the response of the equipment and personnel. Emergencies involving hazardous waste are also coordinated through the Hazards Control Department, but the equipment and personnel in the Toxic Waste Control Group would be activated for large volume waste pumpouts. Native kidney reincarnation following a failed transplant

The aim of this study was to document vascular complications that occurred following cadaveric and living donor kidney transplants in order to assess the overall incidence of these complications at our center as well as to identify possible risk factors. In a retrospective cohort study, 1500 consecutive renal transplant recipients who received a living or cadaveric donor kidney between December 1988 and July 2006 were evaluated. The study was performed at the Nemazee Hospital, Shiraz, Iran. The assessment of the anatomy and number of renal arteries as well as the incidence of vascular complications was made by color doppler ultrasonography, angiography, and/or surgical exploration. Clinically apparent vascular complications were seen in 8.86% of all study patients (n = 133) with the most frequent being hemorrhage (n = 91; 6.1%) followed by allograft renal artery stenosis (n = 26; 1.7%), renal artery thrombosis (n = 9; 0.6%), and renal vein thrombosis (n = 7; 0.5%). ...

Full text: A 51-year-old woman with end stage renal failure secondary to Haemolytic Uraemic syndrome underwent a cadaveric renal transplant. A routine post transplant DTPA scan was performed which demonstrated satisfactory renal transplant perfusion and function. Incidental note was made of tracer uptake in the pelvis in the mid-line, which was suspected to be a uterine fibroid. This was confirmed on ultrasonography and at surgery. One week post transplantation the patient became acutely unwell and at laparotomy a perforated diverticular abscess was drained. Intraoperatively the transplant kidney was examined and the surgeon thought there was a area of infarction. This was confirmed on biopsy. As the patient's creatinine was rising a repeat DTPA study was performed. Perfusion and function of the transplant kidney was virtually absent while Doppler studies showed no flow. Cholangiocarcinoma is synonymous with bile duct carcinoma, and can originate in a small intrahepatic bile duct (peripheral type), a major intrahepatic duct including the hepatic hills, an extrahepatic duct, or near the papilla of Vater (central type). In a sense bile duct carcinoma of the peripheral type is cholangiocarcinoma of the liver; it has the same gross configuration as hepatocellular carcinoma, resulting in difficulty to differentiate on the CT. The authors studied CT findings of 14 cases of pathologically proven peripheral type cholangiocarcinoma of the liver during the last 4 years. The results were as follows: 1. Of 14 cases, 8 were female and 6 were male, and the age ranged from 5th to 7th decades. 2. Asymptomatic cholelithiasis: is cholecystectomy really needed? A critical reappraisal 15 years after the introduction of laparoscopic cholecystectomy.

The goal of the present study was to describe the clinical, haematological and ultrasonographic findings and treatment of 17 cattle with pyelonephritis. Fifteen cattle had an abnormal general condition, which varied in severity; five animals had signs of colic. The urine was brownish-red in 11 animals and cloudy in 13. Clumps of purulent material were seen in the urine of nine animals and clots of blood in two. The specific gravity was lower than normal in 13 animals and ranged from 1.005 to 1.020. A urine test strip revealed protein in 16 animals, blood in 16 and leukocytes in 12. Bacteriological examination of urine yielded Corynebacterium renale in 11 animals, Arcanobacter pyogenes in two and Escherichia coli in one. Asymptomatic cholelithiasis is increasingly diagnosed today, mainly as a result of the widespread use of abdominal ultrasonography for the evaluation of patients for unrelated or vague abdominal complaints. About 10-20% of people in most western countries have gallstones, and among them 50-70% are asymptomatic at the time of diagnosis. Objective was to assess the accuracy of a single versus combined use of ultrasound (US) or computed tomography (CT) in the localization of diseased parathyroid glands. Forty-one patients with hyper-parathyroidism treated surgically between January 2000 to December 2005 at Jordan University Hospital, Amman, Jordan were included in this study. Preoperative ultrasonographic and CT findings were reviewed and compared to the intraoperative and pathologic diagnosis of diseased parathyroid glands. The mean age of patients was 46 years (range 16-70; 15 males and 26 females). Parathyroid adenoma was confirmed in 33 patients and hyperplasia of the parathyroid glands in 8 patients. This document describes the development testing phase associated with the OTA Shear activity and defines the controls to be in place throughout the testing. The purpose of the OTA Shear Program was to provide equipment that is needed for the processing of 40 foot long, sodium wetted, irradiated core components previously used in the FFTF reactor to monitor fuel and materials tests. There are currently 15 of these OTA test stalks located in the Test Assembly Conditioning Station (TACS) inerted vault. The research was related to the effects of endplate thickness and misalignment of fuel bundles in the fuel channel on pressure losses of reactor coolant. The purpose of this invention is to assure operating reliability of a Mandrel head in tubes of various diameters. This goal is achieved in that it is equipped with movable expanding sectors, which are installed on a rigid conical plunger dye. The authors describe the application of an approximate fuzzy C-means (AFCM) clustering algorithm as a data dimension reduction approach to medical magnetic resonance images (MRI). Image data consisted of one T1-weighted, two T2-weighted, and one T2*-weighted (magnetic susceptibility) image for each cranial study and a matrix of 10 images generated from 10 combinations of TE and TR for each body lymphoma study. To quantify the number of cases and clinical courses of Acanthamoeba keratitis treated at the King Khaled Eye Specialist Hospital (KKESH) between December 1982 and May 2005, and to review their clinical course and response to medical and surgical therapy. Retrospective review of medical records of all patients with Acanthamoeba keratitis diagnosed by the KKESH Microbiology or Pathology or Pathology Laboratory since the opening of the hospital. During a period of over 22 years, only six cases of confirmed. Acanthamoeba keratitis were diagnosedand treated at KKESH. In two cases the diagnosis was made by corneal biopsy, while four cases required examination of a histopathology specimen obtained at the time of therapeutic keratoplasty to establish the diagnosis. One important market related capability for firms which seek to develop radical innovations is the competence to involve the 'right' users at the 'right' time in the 'right' form. While former studies have identified a rather passive role of users in the radical innovation process, this paper focuses on the involvement of such users that are in the position to play an active role as inventors and (co)-developers. A multiple case study analysis was conducted in the field of medical technology. Five radical innovation projects within four firms were selected including medical robots and computer-assisted navigation systems. The case study analysis reveals that firms who closely interact with specific users benefit significantly for their radical innovation work. These users have a high motivation toward new solutions, are open to new technologies, possess diverse competencies, and are embedded into a very supportive environment.

The Allocation of Treatment Responsibility scale: A novel tool for assessing patient and caregiver management of pediatric medical treatment regimens

New cassettes for 201 mm{times}252 mm (8{double_prime}{times}10{double_prime}) and 201 mm{times}400 mm (8{double_prime}{times}15.75{double_prime}) storage phosphor plates have been developed at the Synchrotron Resource of the Howard Hughes Medical Institute. The purpose for this work was mainly twofold. Firstly, to diminish the number of manual operations when putting the storage phosphor plate into the cassette or when extracting it from the cassette. Secondly, to render such a cassette much lighter than the former metal cassette previously in use. These two goals were achieved by making new cassettes that are operated as one piece instead of two or three independent parts as with the former systems. The cassettes have been extensively tested and found to be very useful.

Additionally, visual systems impose limits as a result of their high latency, poor image resolution, problems with stereopsis and physical strain upon the user. Audio technology is relatively unexamined in the broad range of available methodologies for medical devices. The potential to translate surgical instrument position into audio feedback presents a novel solution to the human factors and engineering problems faced by visual display technology because audio technology employs a rich and as yet unburdened sensory modality. Causes of morbidity among HIV-infected children on antiretroviral therapy in primary care facilities in Lusaka, Zambia

The performance and characteristics of a medical linear accelerator ''NELAC-1018'' for the electron beam therapy were discussed. Measurement of deep dose distribution revealed that the 80% range was 1.9 cm by 6 MeV, 2.8 cm by 9 MeV, 3.9 cm by 12 MeV, 4.9 cm by 15 MeV and 5.5 cm by 18 MeV. Iso-dose distribution with each tubus and cone showed a favorable flatness by any of the energies. The degree of flatness was extremely favorable, being +-1.7% or less.

Summary Objectives To describe the pattern of incident illness in children after initiation of antiretroviral therapy (ART) in a large public health sector programme in Lusaka, Zambia. Methods Systematic chart review to retrospectively extract data from medical records of children (i.e. <15 years) initiating ART in the Lusaka, Zambia public sector. Incident conditions were listed separately and then grouped according to broad categories. Predictors for incident diagnoses were determined using univariate and multivariable analysis. Biological effects and physical safety aspects of NMR imaging and in vivo spectroscopy

Bacterial adhesion reduction on a biocompatible Si^+ ion implanted austenitic stainless steel

An assessment is made of the biological effects and physical hazards of static and time-varying fields associated with the NMR devices that are being used for clinical imaging and in vivo spectroscopy. A summary is given of the current state of knowledge concerning the mechanisms of interaction and the bioeffects of these fields. Additional topics that are discussed include: (1) physical effects on pacemakers and metallic implants such as aneurysm clips, (2) human health studies related to the effects of exposure to nonionizing electromagnetic radiation, and (3) extant guidelines for limiting exposure of patients and medical personnel to the fields produced by NMR devices. On the basis of information available at the present time, it is concluded that the fields associated with the current generation of NMR devices do not pose a significant health risk in themselves. Feature selection refers to the problem of selecting relevant features which produce the most predictive outcome. In particular, feature selection task is involved in datasets containing huge number of features. Rough set theory has been one of the most successful methods used for feature selection. However, this method is still not able to find optimal subsets. This paper proposes a new feature selection method based on Rough set theory hybrid with Bee Colony Optimization (BCO) in an attempt to combat this. This proposed work is applied in the medical domain to find the minimal reducts and experimentally compared with the Quick Reduct, Entropy Based Reduct, and other hybrid Rough Set methods such as Genetic Algorithm (GA), Ant Colony Optimization (ACO) and Particle Swarm Optimization (PSO).

Large robotized turning centers described

The Nuclear Assurance Corporation (NAC) owns and operates four NAC-1 truck casks. These casks are used to ship spent reactor fuel assemblies and radioactive reactor-core components. The casks have been loaded or unloaded at a total of fifteen nuclear facilities in the United States. In addition, NAC has used another large, overweight-truck cask to ship radioactive reactor core components from a reactor to a waste burial site. There are many individual differences in the cask handling facilities at each of the reactor stations, nuclear research facilities and the storage and burial sites serviced. The introduction of numerical control (NC) machine tools has made it possible to automate machining in series and small series production. The organization of automated production sections merged NC machine tools with automated transport systems. However, both the one and the other require the presence of an operative at the machine for low skilled operations. Industrial robots perform a number of auxiliary operations, such as equipment loading-unloading and control, changing cutting and auxiliary tools, controlling workpieces and parts, and cleaning of location surfaces.
