Introduction: Restrictions in prescribing broad spectrum antimicrobials have been part of a strategy to reduce Clostridium difficile cases in the UK in recent years. However, there has been little work on assessing the safety of alternative antimicrobial agents. Methods: We performed an uncontrolled prospective observational survey over a 1-year period to determine the effectiveness and safety of a new antimicrobial stewardship programme in a district hospital in the UK. Results: In total, 227 Gram-negative bacteraemias (203 episodes) occurred in the study period. No prospective studies have evaluated the effects of correction of iron deficiency anemia on insulin resistance in non-diabetic premenopausal women with iron deficiency anemia. All patients were treated with oral iron preparations. Insulin resistance was calculated with the Homeostasis Model Assessment formula. All patients were dichotomized by the median for age and BMI to assess how the relationship between iron deficiency anemia and insulin resistance was affected by the age and BMI. Although the fasting glucose levels did not change meaningfully, statistically significant decreases were found in fasting insulin levels following anemia treatment both in the younger age (<40 years) (P=0.40) women and in the low BMI (27 kg/m) (P=0.22) subgroups but not in the older age (>= 40 years) and the high BMI (>-27Kg/m) subgroups. ObjectiveHIV and type 2 diabetes are known risk factors for albuminuria, but no previous reports have characterized albuminuria in HIV-infected patients with diabetes.Research Design and MethodsWe performed a cross-sectional study including 73 HIV-infected adults with type 2 diabetes, 82 HIV-infected non-diabetics, and 61 diabetic control subjects without HIV. Serum creatinine >1.5 mg/dL was exclusionary. Oxidative stress is demonstrated to be involved in the pathophysiological mechanism of erectile dysfunction (ED). Quercetin, a potent bioflavonoid, has been reported to have the antioxidant role. In the present study, we examined the effect of quercetin on ED and oxidative stress in streptozotocin (STZ)-induced diabetic rats. Diabetes was induced in Sprague-Dawley rats with a single intravenous injection of STZ. The diabetic rats were then randomized to diabetic group and quercetin therapy groups which were treated with quercetin at different doses of 5, 20 and 50mg/kg per day respectively. At the end of the 8th week, erectile function was assessed by measuring the rise in intracavernous pressure (ICP) following cavernous nerve electrostimulation. We tested the hypothesis that insulin-dependent diabetic patients with coronary artery bypass graft surgery experience altered coupling of cerebral blood flow and oxygen consumption. In a study of 23 patients (11 diabetics and 12 age-matched controls), cerebral blood flow was measured using 133Xe clearance during nonpulsatile, alpha-stat blood gas managed cardiopulmonary bypass at the conditions of hypothermia and normothermia. In diabetic patients, the cerebral blood flow at 26.6 +/- 2.42 degrees C was 25.3 +/- 14.34 ml/100 g/min and at 36.9 +/- 0.58 degrees C it was 27.3 +/- 7.40 ml/100 g/min (p = NS). The control patients increased cerebral blood flow from 20.7 +/- 6.78 ml/100 g/min at 28.4 +/- 2.81 degrees C to 37.6 +/- 8.81 ml/100 g/min at 36.5 +/- 0.45 degrees C (p less than or equal to 0.005). Urinary tract infection (UTI) is a common problem in the female population. Women with diabetes mellitus are possibly at a higher risk. The usual medical therapy for UTI is antibiotic treatment. The aim of this study is to explore strategies employed by diabetic women for prevention and self-treatment of UTI. Forty-two women with diabetes mellitus who had experienced at least one episode of UTI in the last six months were included. Insulitis is considered the histopathological hallmark of type I diabetes. In the non-obese diabetic (NOD) mouse, diabetes has never been observed in the absence of insulitis. The in vivo detection of insulitis could be of relevance for early prediction of diabetes. As approximately 15% of islet-infiltrating lymphocytes express interleukin 2 receptors, the authors have labelled recombinant inter-leukin 2 with "1"2"3I and used this radiopharmaceutical to detect insulitis by gamma camera imaging. The authors studied 71 prediabetic NOD and 27 normal Balb/c mice. Labelled #alpha#-lactalbumin was used as the control protein. Several factors have contributed to this change of position in the UK: the growing realisation that effective action is needed to offset greenhouse gas emissions; higher prices for fossil fuels; increasing reliance on overseas supplies of oil and gas; the limitations of wind and wave power and distribution; security of supply; the gradual realisation in the deregulated electricity generation market that nuclear power is competitive and the pending closure of most of the UK's nuclear fleet within less than 15 years. This report presents estimates of the annual anthropogenic UK emissions of greenhouse gases for the years 1990-93 for submission under the Framework Convention on Climate Change. The gases considered are carbon dioxide, methane, nitrous oxide, nitrogen oxides, carbon monoxide and non-methane volatile organic compounds. Emissions of all the gases have declined since 1990. In particular, those of carbon dioxide have fallen by 4%; those of methane by 8% and those of nitrous oxide by 28%. Emission estimates are reported in the IPCC Standard Tables format disaggregated by source category and fuel. The methodology used for the estimates and how they relate to the UK National Atmospheric Emissions Inventory is discussed in an appendix. (author)

Trace elemental analysis was carried out in various parts of some anti-diabetic medicinal plants using PIXE technique. A 3 MeV proton beam was used to excite the samples. The elements Cl, K, Ca, Ti, Cr, Mn, Fe, Ni, Cu, Zn, Br, Rb and Sr were identified and their concentrations were estimated. The results of the present study provide justification for the usage of these medicinal plants in the treatment of diabetes mellitus (DM) since they are found to contain appreciable amounts of the elements K, Ca, Cr, Mn, Cu, and Zn, which are responsible for potentiating insulin action. Our results show that the analyzed medicinal plants can be considered as potential sources for providing a reasonable amount of the required elements other than diet to the patients of DM. Moreover, these results can be used to set new standards for prescribing the dosage of the herbal drugs prepared from these plant materials.

This invention refers to the organic geochemistry and can be used in prospecting for oil and gas. The method includes taking samples of rocks, extracting from them the chloroform bitumoid and its thermal processing. In order to improve the accuracy, the equal quantities of bitumoid taken are exposed to gradual thermal treatment to temperatures exceeding the thresholds of thermal stability of sulfur-containing compounds, in each temperature interval, the content of hydrogen sulfide released in this case is revealed, and by its drastic increase at temperatures corresponding to the temperatures of the productive part of the section, a conclusion is drawn about the epigenetic nature of the bitumoid. The method makes it possible to improve the resolution of the bituminological prospecting for oil and gas.

There are good prospects for silicon carbide anti-corrosion coatings on fuel elements to be realised, which opens up the chance to reduce the safety engineering requirements to the suitable design and safe performance of the ceramic fuel element. Another possibility offered is combined-cycle operation with high efficiencies, and thus good economic prospects, as with this design concept combining gas and steam turbines, air ingress due to turbine malfunction is an incident that can be managed by the system. This development will allow economically efficient operation also of nuclear power reactors with relatively small output, and hence contribute to reducing CO_2 emissions. (orig./DG).

Investigations have been carried out at the three Corus UK sinter plants over the period 2002-2004 to characterise the emissions of both 2,3,7,8-PCDD/Fs and WHO-12 PCBs, to estimate annual mass releases of these organic micro-pollutants using the I-TEF and WHO-TEF schemes, and to investigate the formation of PCBs in the iron ore sintering process. Results showed that the sintering of iron ore produces a characteristic WHO-12 PCB and PCDD/F congener pattern that is substantially the same for all UK sinter plants. With regard to WHO-12 PCBs, the most abundant congeners were typically PCBs 118 (6-9ngNm-3), 105 (2-4ngNm-3) and 77 (2-3ngNm-3). The study is intended to eventually bring about a number of improvements in the UK electrical system, particularly in regard to Demand Side Management (DSM). Among the benefits envisaged are (a) increased security of supply when the network is under stress; (b) reduction in costs; (c) simpler outage management; (d) carbon saving and (e) increased energy efficiency. So far, these potential benefits have attracted little attention in the residential and small business sectors of the market: suggested reasons for this are listed. Taking into account the experience of other countries, three options are presented for a UK DSM programme: they are (i) customer initiated; (ii) supplier initiated and (iii) distributor initiated. At present, all the suggested options need further study and to aid the development for small customers, five possible initiatives are suggested. Power Fluidic systems for the control of liquids and gaseous flows have been adopted for use in radioactive processing plants in the UK. These devices are intrinsically reliable with no mechanical moving parts because they are able to make use of the hydrodynamics of the fluids being controlled. This reliability feature leads to a zero cell maintenance concept and the elimination of mechanical drive/control systems in cell. The first phase of the development work led to their use in the Fast Reactor Reprocessing Plant at Dounreay and the Highly Active Liquor Storage facility at Sellafield. The success of these early developments has led to an extensive development programme for an extended range of applications in the Thermal Oxide Reprocessing Plant and its associated waste treatment facilities at Sellafield. The technology has now been fully demonstrated and adopted for these plants with considerable benefit over a wide range of applications.

Considerable effort worldwide has been put into trying to find the best way to utilise the large amount of energy that we put into our dustbins every day. One of the problems encountered is that the waste collection systems tend to gather together in one place between 400 and 1000 t of the stuff everyday and since it is possible to extract rather more than 2 MW from each tonne, that is a lot of energy for which to find a market. In continental European cities, especially Germany, and Sweden with most of the population living in apartment blocks, and with colder winters than the UK, district heating provides a suitable market. With its milder winters, more diverse housing and cheap gas the UK is not such a fertile ground for district heating. But since the passing of the Energy Act in 1984, there is a steady market in electricity generation with the advantage that the purchase tariffs are published so that income can be accurately calculated.

This article reports on the operation of Fibrowatt's third chicken waste-fuelled power plant at Thetford in Norfolk, UK. Details are given of the plant which started operation in October 1998, the chicken litter fuel, the spiral screw fuel feeders, the Detroit Stoker grate and spreader stokers which ensure the fuel is burnt mid-air, the production of fertiliser from the ash, and fluegas emission control using cyclones, a baghouse and lime addition to neutralise acid gas. The economics of the project, and the support for the renewable electricity generation under the UK government's Non-Fossil Fuel Obligation are discussed. A site plan of the Thetford plant and a flow diagram of the process are provided.

Determination of the radiocarbon (14C) content of airborne particulate matter yields insight into the proportion of the carbonaceous material derived from fossil and contemporary carbon sources. Daily samples of PM2.5 were collected by high-volume sampler at an urban background site in Birmingham, UK, and the fraction of 14C in both the total carbon, and in the organic and elemental carbon fractions, determined by two-stage combustion to CO2, graphitisation and quantification by accelerator mass spectrometry. OC and EC content was also determined by Sunset Analyzer. The mean fraction contemporary TC in the PM2.5 samples was 0.50 (range 0.27-0.66, n=26).
