This paper reports the characteristics and local management of 826 acute wounds identified during an audit across all health care providers serving the population of Bradford, UK. Of the wounds encountered 303 were traumatic wounds and 237 primary closures with smaller numbers of other acute wound types. Of the 303 traumatic wounds 174 occurred in women (57.4%). Men predominated in the under 45s (65M:26F), this being largely accounted for by hand and finger trauma (n=62) particularly in patients of working age (M32:F12). METHODS Site visit notes, documents, interviews, and online implementation diaries were collected from July 2005 to September 2007 from practice-based research networks (PBRNs) participating in Prescription for Health: Promoting Healthy Behaviors in Primary Care Research Networks (P4H). Plans, engineering parameters, and some test results for several FFTF core and primary sodium circuit instrument systems are presented. The systems discussed include temperature, flow, pressure, leak detectors, level sensors, fuel failure monitoring, sodium impurity analysis and cover gas monitors. Since many of these instruments are similar to those used in other fast reactors around the world, only a brief description is presented for these systems. Following the February 2011 earthquake in Christchurch, New Zealand (NZ), the authors participated in counselling local residents, and debriefing and supervising support teams. Indications were that risk for mental health disorders, including Post-Traumatic Stress Disorder (PTSD), may be elevated in residents, and that this risk may continue for some time. Patients may be de-prioritising their mental health issues when these become normalised throughout the city's population. The authors recommend that primary care patients are assessed using a brief, comprehensive tool (for example, the Case-finding and Help Assessment Tool) that targets many health and behavioural issues identified as increasing in the city following the earthquake. We show that the HACCP system is not fully applicable at the primary production level, and that food safety is obtained through the careful implementation of good hygiene practice (GHP) at the farm. This study aimed to reveal that severe disturbance of attachment relationship with primary care-giver can affect functional and anatomical brain development by measuring cerebral perfusion on "9"9"mTc-ECD brain SPECT and correlative MRI. We included 18 children aged 31 to 76 months who met the diagnostic criteria of RAD as defined in DSM-IV and ICD-10 and SSP and CARS. "9"9"mTc-ECD SPECT was performed using CERASPECT. MRI was performed in all patients. SPECT data were visually assessed. 15 of 18 children had abnormal perfusion on SPECT, revealing decreased perfusion of Lt.thalamus (7/15) and Rt.thalamus (3/15), and bilateral thalami (5/15). Digital radiography for diagnosis of chest diseases using Fuji Computed Radiography (FCR) was evaluated. The results were as follows; 1. Compared to the conventional chest X-ray films, various types of image enhancement could be made by manipulation of the digital information such as tone conversion, spatial frequency modification etc. 2. Digital radiography lessens the X-ray exposure dose and will ultimately permit speedy transmission of image data from outlying clinics to central processing hospitals. 3. Digital radiographic images are useful in chest X-ray diagnosis especially in mass surveys for lung cancer and in primary care medicine because of its full imaging capability. (author).

Assessment of primary impacts of a hydrogen economy in New Zealand using UniSyD

Technological advances and applications of geothermal energy pile foundations and their feasibility in Australia

Small economies such as New Zealand risk significant economic hardship without careful evaluation of alternatives to petroleum-based transportation due to the adverse effects of climate change and depleting international oil reserves. This paper uses an integrated multi-regional multi-fleet system dynamics model of New Zealand's energy economy to assess the primary impacts of alternative vehicle fleet technologies. Results suggest that hydrogen fuelled HICEs and FCVs may offer significantly greater economic savings than BEVs due to a much lower capital cost. Under our Base Case, 65% of the light fleet are HICEs and FCVs and 5% BEVs. Excluding hydrogen vehicles from the vehicle fleet can result in an average annual cost of US$562 per vehicle between 2015 and 2050. Co-production of hydrogen and electricity using coal gasification with carbon capture and storage is the dominant long term hydrogen production technology. (author)

The Northwest Foster Care Alumni Study examined the relation between experiences in foster care and depression among young adults who spent at least a year in foster care as adolescents. Results indicate that preparation for leaving foster care, nurturing supports from the foster family, school stability, access to tutoring, access to therapeutic service and support, and a shorter and more stable placement history were associated with a lower probability of depression in young adulthood. Validation of the Simplified Acute Physiology Score 3 scoring system in a Korean intensive care unit.

This study examined whether cognitive behavioural therapy (CBT) could prevent the development or worsening of panic-spectrum psychopathology and anxiety symptoms in chronic obstructive pulmonary disease (COPD). 41 patients with COPD, who had undergone pulmonary rehabilitation, were randomised to either a four-session CBT intervention condition (n = 21) or a routine care condition (n = 20). Assessments were at baseline, post-intervention, and at 6-, 12- and 18-month follow-ups. Primary outcomes were the rates of panic attacks, panic disorder and anxiety symptoms. Secondary outcomes were depressive symptoms, catastrophic cognitions about breathing difficulties, disease-specific quality of life and hospital admission rates. There were no significant differences between the groups on outcome measures at baseline. During the second half of 1993, the groundwater at the F-Area Seepage Basins (FASB) was monitored in compliance with Module 3, Section C, of South Carolina Hazardous Waste Permit SC1-890-008-989, effective November 2, 1992. The monitoring well network is composed of 87 FSB wells screened in the three hydrostratigraphic units that make up the uppermost aquifer beneath the FASB. A detailed description of the uppermost aquifer is included in the Resource Conservation and Recovery Act Part B post-closure care permit application for the F-Area Hazardous Waste Management Facility submitted to the South Carolina Department of Health and Environmental Control (SCDHEC) in December 1990. Reliability analysis of FPSO mooring systems and the interaction with risers

Radio-biologists have pointed out a novel radiobiological phenomenon observed in many tumor and normal cell lines: hyper-radiosensitivity to very low-dose (H.R.S.) followed by induced radioresistance (I.R.R.) after a threshold dose of 0.1-0.3 Gy that depends on the cell line. Radioresistance at high dose (i.e. higher than 0.5 Gy) and metastatic potential of tumor cells are likely major factors of failure in radiotherapy. A careful review of literature suggests that: 1) radiotherapy does not increase the metastatic potential of tumor cells; 2) radioresistance at high dose and metastatic potential are not related. However, inside a given tumor cell line, highly metastatic clones may elicit more cells showing H.R.S; or are more radiosensitive at high dose than poorly metastatic ones. The current mooring codes do not have a consistent set of requirements; for example, the intact mooring system safety factors range from 1.5 to 2.3. This report presents estimates of the annual anthropogenic UK emissions of greenhouse gases for the years 1990-93 for submission under the Framework Convention on Climate Change. The gases considered are carbon dioxide, methane, nitrous oxide, nitrogen oxides, carbon monoxide and non-methane volatile organic compounds. Emissions of all the gases have declined since 1990. In particular, those of carbon dioxide have fallen by 4%; those of methane by 8% and those of nitrous oxide by 28%. Emission estimates are reported in the IPCC Standard Tables format disaggregated by source category and fuel. The methodology used for the estimates and how they relate to the UK National Atmospheric Emissions Inventory is discussed in an appendix. (author)

Abstract Aims To determine the long-term health economic benefits associated with lispro vs. regular human insulin (RHI) in UK Type 1 diabetic (T1DM) patients using the previously published and validated CORE Diabetes Model. Methods A literature review designed to capture clinical benefits associated with lispro and T1DM cohort characteristics specific to UK was undertaken. Clinical benefits were derived from a Cochrane meta-analysis. The estimated difference (weighted mean) in glycated haemoglobin (HbA1c) was -0.1% (95% confidence interval -0.2 to 0.0%) for lispro vs. RHI. The teaching of high energy physics in British universities

The health politics and policy communities are still struggling with the question of "what went wrong" in the 1993-94 health care reform effort. Here I identify which Americans were politically active and inactive during the health care reform debate to explore the role political participation may have had in determining the outcome of the debate. Using data from a national and California random-sample telephone surveys, and controlling for other demographic attributes, I found that those who engaged in political activity specifically related to health care reform were disproportionately more likely to be self-identified conservatives, less likely to favor an employer mandate plan, more likely to be fifty to sixty-four years old, more likely to be men, and more likely to have greater interest in and knowledge of the health care issue. An analysis is given of a survey of the teaching of high energy physics in British universities. The subject changes quickly, and there is a continual conflict between new and old material. Different courses may deal with this in different ways. To find out what is actually being taught to students, details were obtained from all 50 university physics departments in the United Kingdom (UK) by means of a questionnaire. This covered the course structure - whether it was optional or compulsory or contained both elements - the number of lectures given, and the topics covered in the syllabus. 2000 MW HVDC link between England and France

A wind energy study (WEST) in the coastal region of Bangladesh has been undertaken, in collaboration with Local Government Engineering Department (LGED) and with technical support of the UK`s Energy Technology Support Unit (ETSU) under an ODA programme. Seven 25m high masts have just been installed at selected places and data for a complete year will be recorded. A brief account of the mast erection is given and some preliminary data is presented. Investigations have been carried out at the three Corus UK sinter plants over the period 2002-2004 to characterise the emissions of both 2,3,7,8-PCDD/Fs and WHO-12 PCBs, to estimate annual mass releases of these organic micro-pollutants using the I-TEF and WHO-TEF schemes, and to investigate the formation of PCBs in the iron ore sintering process. Results showed that the sintering of iron ore produces a characteristic WHO-12 PCB and PCDD/F congener pattern that is substantially the same for all UK sinter plants. With regard to WHO-12 PCBs, the most abundant congeners were typically PCBs 118 (6-9ngNm-3), 105 (2-4ngNm-3) and 77 (2-3ngNm-3). Biological responses to the chemical recovery of acidified fresh waters in the UK

We report biological changes at several UK Acid Waters Monitoring Network lakes and streams that are spatially consistent with the recovery of water chemistry induced by reductions in acid deposition. These include trends toward more acid-sensitive epilithic diatom and macroinvertebrate assemblages, an increasing proportional abundance of macroinvertebrate predators, an increasing occurrence of acid-sensitive aquatic macrophyte species, and the recent appearance of juvenile (<1 year old) brown trout in some of the more acidic flowing waters. Changes are often shown to be directly linked to annual variations in acidity. Although indicative of biological improvement in response to improving water chemistry, 'recovery' in most cases is modest and very gradual. The study is intended to eventually bring about a number of improvements in the UK electrical system, particularly in regard to Demand Side Management (DSM). Among the benefits envisaged are (a) increased security of supply when the network is under stress; (b) reduction in costs; (c) simpler outage management; (d) carbon saving and (e) increased energy efficiency. So far, these potential benefits have attracted little attention in the residential and small business sectors of the market: suggested reasons for this are listed. TRADE AND ECONOMIC GEOGRAPHY: THE IMPACT OF EEC ACCESSION ON THE UK*

Power Fluidic systems for the control of liquids and gaseous flows have been adopted for use in radioactive processing plants in the UK. These devices are intrinsically reliable with no mechanical moving parts because they are able to make use of the hydrodynamics of the fluids being controlled. This reliability feature leads to a zero cell maintenance concept and the elimination of mechanical drive/control systems in cell. The first phase of the development work led to their use in the Fast Reactor Reprocessing Plant at Dounreay and the Highly Active Liquor Storage facility at Sellafield. The success of these early developments has led to an extensive development programme for an extended range of applications in the Thermal Oxide Reprocessing Plant and its associated waste treatment facilities at Sellafield. The technology has now been fully demonstrated and adopted for these plants with considerable benefit over a wide range of applications.

We combine establishment-level employment data with trade data by port to examine the impact of accession to the European Economic Community on the spatial distribution of UK manufacturing. We test the predictions from economic geography models of how external trade affects the spatial distribution of employment. We treat accession as a natural experiment which helps mitigate problems of endogeneity. Our results suggest that accession changed the country composition of trade and via the port composition induced an exogenous shock to different locations. In line with theory, we find that better access to export markets and intermediate goods increase employment while increased import competition decreases employment.

Sizewell B is Nuclear Electric's latest power station, and the Pressurised Water Reactor (PWR) design on which it is based represents a ''first'' for the UK. One of the integral components of the plant is the heating, ventilation and air-conditioning (HVAC) system, which performs a contamination control and gaseous waste management function for the site. During the commissioning of Sizewell B Power Station the extract systems of the HVAC plant underwent a procedure known as ''fingerprinting''. Employment, job turnover, and trade in producer services: UK firm-level evidence

Considerable effort worldwide has been put into trying to find the best way to utilise the large amount of energy that we put into our dustbins every day. One of the problems encountered is that the waste collection systems tend to gather together in one place between 400 and 1000 t of the stuff everyday and since it is possible to extract rather more than 2 MW from each tonne, that is a lot of energy for which to find a market. In continental European cities, especially Germany, and Sweden with most of the population living in apartment blocks, and with colder winters than the UK, district heating provides a suitable market. With its milder winters, more diverse housing and cheap gas the UK is not such a fertile ground for district heating. But since the passing of the Energy Act in 1984, there is a steady market in electricity generation with the advantage that the purchase tariffs are published so that income can be accurately calculated.

Abstract We provide the first firm-level evidence of the impact of the trade in producer services (-offshoring-) on the labour market. Using a new data set from the UK that measures trade in services at the firm level, we find no evidence that importing intermediate services is associated with job losses or greater worker turnover. Using regression to control for observable differences between firms that import service inputs and those that do not, we show that firms that start importing intermediate services experience faster employment growth than equivalent firms that do not. This seems likely to be the result of positive demand shocks, which cause a simultaneous increase in employment, output, and use of imported service inputs.

This article reports on the operation of Fibrowatt's third chicken waste-fuelled power plant at Thetford in Norfolk, UK. Details are given of the plant which started operation in October 1998, the chicken litter fuel, the spiral screw fuel feeders, the Detroit Stoker grate and spreader stokers which ensure the fuel is burnt mid-air, the production of fertiliser from the ash, and fluegas emission control using cyclones, a baghouse and lime addition to neutralise acid gas. The economics of the project, and the support for the renewable electricity generation under the UK government's Non-Fossil Fuel Obligation are discussed. A site plan of the Thetford plant and a flow diagram of the process are provided.

Application of 14C analyses to source apportionment of carbonaceous PM2.5 in the UK

Following the United Kingdom's nuclear tests at Maralinga in Australia in the 1960's, the Australian government signed away its rights to claim compensation for decontamination costs in a 1968 agreement. Controversy now rages as the Australian government is claiming pound33 million for this purpose from the United Kingdom to compensate the Maralinga Tjarutja for the loss of access to their traditional lands. They claim that the UK clean-up operation was inadequate and that much plutonium was spread over the land, rather than contained in pits. Measurements of contamination at the site were wrong by a factor of ten according to a 1984 visit when Australian scientists were horrified to realize that an Aboriginal child living near Taranaki could inhale more than 460 millisieverts a year. There are a total of nine operational nuclear plants in Spain totalling 7.350 MWe. These units produced 54.265 x 106 KWh in 1990, 36% of the total generation in Spain. Seven of these plants are of the PWR type. The first plant in operation was Jose Cabrera (ZORITA) in 1968, one loop Westinghouse plant with a model 24 Steam Generator. Due to the design margin and careful operation of the Steam Generator of this plant its performance have been very good, with only 5% tubes plugged after 23 years of operation. We investigate opportunistic cooperation between unlicensed secondary users and legacy primary users in a cognitive radio network. Specifically, we consider a model of a cognitive network where a secondary user can cooperatively transmit with the primary user in order to improve the latter's effective transmission rate. For the performance assessment of radioactive and hazardous waste disposal sites, risk assessments are usually performed for the long term, i.e., over an interval in space and time for which one can predict movement and behavior of toxic agents in the environment. This approach is based on at least three implicit assumptions: One, that the engineering layout will take care of the immediate endangerment of potential receptors; two, that one has carefully evaluated just how far out in space and time the models can be extrapolated, and three, that one can evaluate potential health effects for very low exposures. The quark structure of constituent nucleons is considered by a qualitative comparison of features of different models with experimental data with respect to cumulative meson production. This is an account of the initial struggles and subsequently successful career of a young woman growing up in prewar Australia with the overriding desire to become a physicist despite various problems and prejudices. The book provides first-hand accounts of wartime radar research and development in Australia; the immediately post-war era in nuclear physics at the Cavendish Laboratory, Cambridge; and work in the Nuclear Physics Division at AERE Harwell in the period from the 1950s onwards. Therapy of Primary Breast Cancer

ICU telemedicine uses audiovisual conferencing technology to provide critical care from a remote location. Research is needed to best define the optimal use of ICU telemedicine, but efforts are hindered by methodological challenges and the lack of an organized delivery approach. We convened an interdisciplinary working group to develop a research agenda in ICU telemedicine, addressing both methodological and knowledge gaps in the field. To best inform clinical decision-making and health policy, future research should be organized around a conceptual framework that enables consistent descriptions of both the study setting and the telemedicine intervention. The framework should include standardized methods for assessing the preimplementation ICU environment and describing the telemedicine program. The shape of their energy spectrum depends on the primary cosmic ray spectrum incident at the top of the atmosphere. At high energies, the spectral shape depends on the primary spectrum of helium and heavy nuclei. However, at very low energies, specially below the geomagnetic cut-off, the spectral shape depends on the evaporation and recoil processes and hence almost independent of the spectral shape of the primary radiation. Use of primary electron counting for detection of ultrasoft x-rays with a low-pressure multistep gaseous detector

The relative merits and difficulties of the primary and secondary origin hypotheses for the observed cosmic ray antiprotons, including the low energy measurement of Buffington, were examined. It is concluded that the cosmic ray antiproton data may be strong evidence for antimatter galaxies and baryon symmetric cosmology. The present antiproton data are consistent with a primary extragalactic component having antiproton/proton approximately equal to .0032 + or - 0.7.

Detailed chemical kinetic reaction mechanisms for primary reference fuels for diesel cetane number and spark-ignition octane number

The use of primary electron counting techniques as an alternative to the more usual parallel plate avalanche chamber that has been employed in soft x-ray scattering experiments is being investigated at the National Synchrotron Light Source. The theoretical aspects of primary electron counting and motivation behind building a primary electron counting detector are described, as well as characteristics and future improvements of the device constructed at the NSLS. The detector consists of a low electric field drift region and a low pressure multistep avalanche region which can be operated with two or three stages of electron multiplication. The device has worked well in extensive tests as a simple parallel plate avalanche chamber, providing energy resolutions of 58% and 43% at 277 and 500 eV, respectively. This paper reports that more than 800 miles of pipeline will come into service on the U.K. continental shelf during 1993, surpassing by 75% the previous record set in 1977. About 3,500 miles of pipeline is in place. Estimation of seasonal correction factors through Fourier decomposition analysis-a new model for indoor radon levels in Irish homes

Over the last four decades the Irish Sea has received controlled discharges of radioactive effluents from the Sellafield (Windscale) nuclear fuel reprocessing plant in Cumbria, UK. Enhanced levels of a range of fission, activation and transuranic elements have been reported in a variety of environmental media. Most of the {sup 239,240}Pu and {sup 241}Am and about 10% of the {sup 137}Cs have been retained in a deposit of the fine sediment near the discharge point. The quantities of radionuclides discharged annually from Sellafield decreased by two orders of magnitude from the mid-1970s to 1990, but estimated critical group internal and external exposure decreased by less than one order of magnitude over this period.
