Salivary cortisol assessment in the evaluation of hypothalamic-pituitary-adrenal function

A simple, short and sensitive direct radio-immunoassay technique for the determination of salivary cortisol concentration was employed to assess saliva as a medium for evaluating cortisol response during endocrine testing in 9 controls and 40 patients. Results in controls suggested that an adequate salivary cortisol response to insulin hypoglycaemia was an increase of 150% above the basal value with a minimum peak of 15 nmol/l. Thirty-three patients were classified as being either good or poor responders to insulin hypoglycaemia on the basis of criteria for plasma cortisol levels. When the defined salivary cortisol response was used for assessment, all 33 patients were correctly categorized into the same response groups. The salivary cortisol response to intramuscular tetracosactin in 3 patients and an intravenous dexamethasone infusion in 4 patients confirmed the value of saliva as an assay medium. ...

The aim of this study was to evaluate the radioprotective effect of turmeric extract (40 mg/kg body weight) and vitamin E (#alpha# - tocopherol acetate, 400 IU/kg body weight) supplementation on lipid peroxidation, reduced glutathione and antioxidant defense enzymes in various organs like liver, kidney and salivary glands at 24 h in adult Swiss mice. "1"3"1Iodine exposure significantly increased lipid peroxidation in kidney and salivary glands in comparison to control animals. Pre supplementation with turmeric extract for 15 days showed significant lowering of lipid peroxidation in kidney. On the other hand vitamin E pre supplementation showed marked reduction in lipid peroxidation in salivary glands. Reduced glutathione levels decreased significantly in liver after radiation exposure. For the evaluation of diseases of salivary origin, ultrasound and/or sialography is recommended instead of MRI. In cases of salivary tumor, ultrasound may be helpful in delineating superficial tumors. For small tumors no further imaging is needed. When dealing with a large tumor or a tumor in the deep lobe of the parotid gland, MRI is the preferred imaging method. Not only does MRI provide a large variety of soft tissue signal differences, but also the multiplanar facilities are helpful in delineating the extent of the tumor, whether located in the submandibular, sublingual, or parotid gland. Skull base invasion is often well seen by MRI. Subtle changes may be missed and in those cases CT is recommended to exclude or prove destruction of the skull base. Optimum aspect ratio for heat transfer enhancement in curved rectangular channels

Enhanced heat transfer in curved rectangular channels depends on at least two geometric factors, the radius of curvature of the duct and the aspect ratio, which is the width/depth ratio of the duct. In this work, an expression for optimum aspect ratio is derived mathematically from a correlation that includes both aspect ratio and bend curvature parameters. Enhancement factors are developed that show optimal, near-optimal, and nonoptimal aspect ratios for curved rectangular ducts. Predicted enhancements are within 25% of published literature results for individual designs, two of which show heat transfer enhancement of 200% over that of a straight duct with turbulent flow. The theoretical maximum enhancement is shown to be about 480% over a straight-duct, turbulent-flow situation. Systemic and splanchnic hemodynamics of the chronic bile duct-ligated rat were characterized by radioactive microspheres. Conscious and pentobarbital sodium-anesthetized, bile duct-ligated and sham-operated rats had cardiac output and regional organ blood flows determined. The conscious bile duct-ligated rat compared with the sham-operated showed a hyperdynamic circulation with an increased cardiac output and portal tributary blood flow. Pentobarbital sodium anesthesia induced marked hemodynamic changes in both sham-operated and bile duct-ligated rats. Thermally developing flow in curved square ducts with internal fins

The laminar incompressible hydrodynamically fully developed and thermally developing flow is studied in a curved square duct with four longitudinal fins. The duct is successively subjected to constant wall temperature, to circumferentially uniform temperature and axially linearly or exponentially varying temperature. The local and fully developed Nusselt numbers are examined for various values of the Dean number and it is found that the heat transfer rate increases for high fins. The parameters that affect the entry length are studied and the fluctuations of the local Nu that appear in the entrance region are investigated. Temperature contour plots are presented for the visualization of the temperature field and functional relations for the Nusselt number are proposed in terms of the Dean and Prandtl numbers. (orig.)

Optimum profiles for asymmetrical longitudinal fins in annular ducts

In the present work the geometry of annular ducts with asymmetrical longitudinal fins is optimized in order to enhance the heat transfer under laminar coolant flow conditions. The heat transferred is also maximized for a given amount of material or hydraulic resistance. Polynomial profiles are assigned to the two lateral fin surfaces. Velocity and temperature distributions on the annular duct cross section are determined with the help of a finite-element model. A global heat transfer coefficient and an equivalent Nusselt number are then calculated. Lastly, optimum asymmetrical fins obtained by means of a genetic algorithm are shown for different situations and their performance is compared with those of optimum symmetrical fins.

Safety designs for sludge ducts in brown coal briquetting plants

Studies technological safety of installing a water spray pressure vessel between electrostatic dedusters and coal sludge ducts. These sprays are in use elsewhere for steam generator ash removal. Dust ignition and explosion tests were carried out to examine flame and pressure wave propagation through the vessel into ducts. Water jet diameter, amount of water sprayed and coal dust removed were varied. Pressure waves exceeded 250 Pa. Test results show the vessel to be suitable for installation in briquetting plants due to its flame and explosion barrier effect and extermination of smoldering dust fires. The only disadvantage of the vessel is seen as its water and electric power consumption; about 8/sup 3//h of water and 1.5 kW/h of power per vessel serving dedusters of a 2,200 m/sup 2/ rotary brown coal dryer.

Magnetic resonance (MR) images of 18 cases of hilar cholangiocarcinoma were evaluated to compare the effectiveness of Gd-DTPA with that of high dose contrast enhancement computed tomography (HCE-CT) in detecting the primary tumor. The primary tumor was demonstrated as having slightly low intensity compared with liver parenchyma and high intensity compared with the delated bile duct on T1-weighted images. In contrast, MRI using Gd-DTPA, which was carried out in five cases, revealed intense enhancement of the tumor. As the differentiation between cholangiocarcinoma and dilated bile duct was difficult, it was concluded that the use of Gd-DTPA improves the efficacy of MRI in diagnosing cholangiocarcinoma. This report describes in detail the First Generation Duct Injection Model developed for the analysis of the fundamental processes of flue gas desulfurization by sorbent injection. The model is a two- and three-dimensional, multiphase reacting flow analyzer using computational fluid dynamics methods. The gaseous phase is solved in an Eulerian frame while the droplets or particles are tracked in a Lagrangian frame. A radial-flow type internally oil-cooled oil-filled cable has been developed. This system is characterized in that the oil flows radially through the paper insulation. The thermal resistance of the insulation is equivalently decreased by the radial-flow. The decreased thermal resistance as well as the cooling effect of the oil flow in the central oil duct and the outer oil duct under an aluminum sheath increase the current capacity. A calculation method of cooling characteristics is introduced. The stability of the cable and accessories was confirmed through initial electrical tests and a long-term field test.

Computed tomography of hilar cholangiocarcinoma: a new sign

Thirty-seven patients with histologic proof of cholangiocarcinoma at the confluence were examined by computed tomography (CT) to determine whether this examination is of value in the assessment of these patients for surgery and whether there are any features specific to this type of tumor. Thirty-two patients showed intrahepatic duct dilatation; six of these showed dilatation of ducts in one lobe only. Eighteen patients had intrahepatic low-attenuation areas, while eight had a mass lesion in the porta hepatis. The results of this study show that CT provides useful anatomic information preoperatively but that the appearances are nonspecific. Lobar atrophy is highly suggestive of hilar cholangiocarcinoma, either of long-standing or with unilateral portal venous involvement.

In the self-cooling blankets of the Tokamak fusion reactor, a liquid metal, namely liquid lithium, is pumped through a system of ducts to transfer heat and capture neutrons. One of the blanket designs proposed in Argonne National Laboratory's Blanket Comparison and Selection study uses a combination of poloidal and toroidal ducts in order to maximize heat transfer while minimizing net pressure drop. To assess the CT findings of intrahepatic cholangiocarcinoma associated with hepatolithiasis. The CT features of 26 patients with cholangiocarcinoma and hepatolithiasis were reviewed and compared with those of 23 patients with intrahepatic stones alone, acting as control subjects. CT findings were analyzed for tumor appearance and adjacency to hepatolithiasis. We studied clinical findings, noting the presence or absence of wall thickening or soft tissue attenuation within the bile duct, the luminal diameter of dilated bile duct, and the presence of periductal enhancement, and compared these with the findings for control groups. Radioiodide uptake in brain, CSF, thyroid, and salivary glands of audiogenic seizure mice

DBA/2J (DBA) mice are susceptible to audiogenic seizures (ASs) in an age-dependent manner. Anion transport as measured by radioiodide uptake was determined in thyroid gland, salivary gland, skeletal muscle, cerebral cortex, cerebellum, brainstem, and CSF from these mice at various ages. Anion transport was also determined in C57BL/6J(C57) mice, an AS-resistant strain. In thyroid, DBA mice had an enhanced ability to concentrate iodide at 21 days of age when they have maximal AS susceptibility, as compared with the same-aged C57 mice. This difference in thyroid function was less marked at 40 days of age, when DBA mice are less AS susceptible, and was absent at 110 days of age, when DBA mice are AS resistant. In such cases, remedial action can be taken by creating ductless longitudinal ventilation. Methods: Sixty-four patients with hepatic hilar cholangiocarcinomas confirmed by surgery or pathology underwent MRI using a 1.5-T superconductive MR system including conventional unenhanced MRI, MRCP and dynamic contrast-enhanced MRI with Gd-DTPA. Analysis of the placement of multiple metallic stents in the treatment of hilar cholangiocarcinoma

Objective: To evaluate the clinical efficacy of multiple stents placement in the management of hilar cholangiocarcinoma, especially in the complex cases of which the hepatic ducts are invaded. Methods: Forty-five consecutive patients with hilar cholangiocarcinoma were treated with percutaneous transhepatic placement of two or three self-expandable metallic endoprostheses. The cause of hilar obstructions in these patients were all cholangiocarcinoma, including Bismuth classification type II (n 12), IIIa (n 17), IIIb (n 10), and IV (n 6). My assignment on the SP-100 Vacuum Vessel Vacuum System Team was to perform a transient pump down analysis for the vacuum vessel that will house the SP-100 reactor during testing. Pump down time was calculated for air and helium. For all cases the proposed vacuum system will be able to pump down the vessel within the required time. The use of a larger rotary piston pump (DUO250) improves the pump down time by 35 minutes and therefore should be considered. The 6-inch duct for the roughing line is optimal, however, because all cases are well below the 24 hour time frame, the 4-inch duct is sufficient. The use of the single turbomolecular pump during pump down is sufficient. A pump down with helium in the vessel and a helium inleakage delays the time to achieve the base pressure marginally and is acceptable.

Treatment of hilar cholangiocarcinoma with inserting biliary double stents

Objective: To investigate the inserting technique of biliary double stents in treating hilar cholangiocarcinoma. Methods: 6 patients with hilar cholangiocarcinoma (Bismuth IV) were treated by percutaneous transhepatic insertion of biliary stents. Double stents were inserted in each patient. Different inserting methods were adopted according to the branch angles formed by left and right hepatic ducts. Results: The jaundice of all patients alleviated or disappeared obviously after stent implantation. The average difference between post-and pre-operation in the serum total bilirubin level was (104 #+-# 29) #mu#mol/L (P<0.01). Stent obstruction was found in 2 cases after 4 and 6 months respectively. Conclusion: Double stents implantation is effective for the treatment of hilar cholangiocarcinoma. The authors study technological safety of installing a water spray pressure vessel between electrostatic deduster and coal sludge ducts. These sprays are in use elsewhere for steam generator ash removal. Dust ignition and explosion tests were carried out to examine flame and pressure wave propagation through the vessel into ducts. Water jet diameter, amount of water sprayed and coal dust removed were varied. Pressure waves exceeded 250 Pa. Test results show the vessel to be suitable for installation in briquetting plants due to its flame and explosion barrier effect and extermination of smoldering dust fires. The only disadvantage of the vessel is seen as its water and electric power consumption: about 8 m/sup 3//h of water and 1.5 kW/h of power per vessel serving dedusters of a 2,200 m/sup 2/ rotary brown coal dryer. The Thomsen-Friedenreich (T) antigen, #betta#-D-Gal-(1->3)-#alpha#-D-GalNAc, is exposed in reactive form on many human adenocarcinomata, but not on corresponding benign tissues. Peanut lectin (PNA) has a strong binding affinity for the T antigen and reportedly binds preferentially to certain malignant tissues. We investigated the potential of radio-labelled PNA as a tumour localising agent in an animal model system using a mouse lymphoma (RI) shown to bind fluorescein-labelled PNA in vitro. The radio-iodinated lectin showed good tumour localisation and rapid blood clearance. Clear images of tumours were obtained, in serial scintigraphic imaging, by 24 and 48 h. No blood background subtraction was necessary. The human CDC25 tyrosine phosphatases trigger activation of CDC2 by removing inhibitory phosphates; thus the genes encoding these phosphatases may be suspected as potential oncogenes due to their role in promoting cell division. To date, three human CDC25 genes have been identified: CDC25A, B, and C. This communication describes the mapping of CDC25A to chromosome 3p21 and CDC25B to chromosome 20p13 by fluorescence in situ hybridization with confirmation by the polymerase chain reaction of hamster-human somatic cell hybrid DNA. 3p21 is near an area frequently involved in karyotypic abnormalities in renal carcinomas, small cell carcinomas of the lung, and benign tumors of the salivary gland. 20p13 does not seem to be a common area for karyotypic alteration in tumors. Variation in the sensitivity of the mouse spermatogonial stem cell population to fission neutron irradiation during the cycle of the seminiferous epithelium

Dose-response studies of the radiosensitivity of spermatogonial stem cells in various epithelial stages after irradiation with graded doses of fission neutrons of 1 MeV mean energy were carried out in the Cpb-N mouse. These studies on the stem cell population in stages IX-XI yielded simple exponential lines characterized by an average D0 value of 0.76 +/- 0.02 Gy. In the subsequent epithelial stages XII-III, a significantly lower D0 value of 0.55 +/- 0.02 Gy was found. In contrast to the curves obtained for stem cells in stages IX-III, the curves obtained in stages IV-VIII indicated the presence of a mixture of radioresistant and radiosensitive stem cells. The solid-pseudopapillary tumor (SPT) of the pancreas is a rare type of exocrine pancreatic neoplasm. SPT predominantly affects young women and female children, and is usually discovered incidentally. This tumor is generally benign with a low incidence of malignancy. A 13-year-old girl was admitted to the hospital with a few weeks' history of mild abdominal pain and jaundice. On physical examination, there was no palpable mass. The primary aim was to evaluate delayed contrast-enhanced MRI in depicting perineural spread of hilar cholangiocarcinoma (CCC) and consequently to determine the capability of MRI/MRCP for staging CCC. Fifteen patients that underwent MRI/MRCP and surgical treatment were retrospectively included. Two radiologists evaluated MR images to assess delayed periductal enhancement, extent of bile duct stenosis, liver parenchymal and vascular involvement and presence of liver atrophy. An agreement between delayed enhancement of the bile duct walls and perineural neoplastic spread showed a very good correlation factor (0.93). Laser holographic interferometry and pressure measurements are presented for the effects of rib-to-duct height ratio (H/2B), rib pitch-to-height ratio (Pi/H), and Reynolds number (Re) on the spatially periodic-fully developed turbulent heat transfer and friction in a rectangular duct of width-to-height ratio of 4:1 with an array of ribs detached from one wall at a clearance to rib-height ratio of 0.38. The ranges of H/2B, Pi/H, and Re examined were 0.13 to 0.26, 7 to 13, and 5 {times} 10{sup 3} to 5 {times} 10{sup 4}, respectively. However, it is only symptomatic in 20-30% of patients, with biliary pain "colic" being the most common symptom. Complications of asymptomatic gallstone disease are generally rare, with an incidence of <1 %/yr. The most common complications of gallstone disease are acute cholecystitis, acute pancreatitis, ascending cholangitis, and gangrenous gallbladder. Less frequent complications include Mirizzi syndrome, cholecystocholedochal fistula, and gallstone ileus. Decontamination operations included dismantling and removing gloveboxes and conveyor tunnels; removing process systems, utilities, and exhaust ducts; and decontaminating all remaining surfaces. Improvement of the efficiency of a bare solar collector by means of turbulence promoters

Results obtained by previous authors for heat transfer and friction in rectangular ducts with periodic disturbances were employed to evaluate the effect of such disturbances on bare collector efficiency and on its pressure drop. An effective efficiency, which takes into account extra heat obtained and additional mechanical (electric) energy consumption for moving air when heat-transfer augmentation devices are employed, was defined. Forty-one patients with uncomplicated gall stone disease were laparoscopied with the object of cholecystectomy. The procedure was accomplished in 36 patients, but the operation had to be transformed to a conventional open operation in five: Fibrosis made dissection of the gall bladder hazardous in four and bleeding during the procedure made immediate laparotomy necessary in one patient, whose postoperative course was uneventful. The median operating time was 100 minutes, range was 60-250 minutes. The only operative complication was bleeding from a trocar puncture hole on the first postoperative day which stopped spontaneously in one patient. Eighteen were sent home on the first postoperative day and 12 patients on the second day. Peroperative cholangiography was performed employing the Olsen-Reddick cholangiography forceps. Mitotic delay is described as a classical response to radiation; however, circadian rhythmicity in cell division in vivo has not been considered by many authors. The present study investigated the relation between fluctuations reported as mitotic delay and recovery in vivo and circadian oscillations in mitotic index in mouse corneal epithelium. One aspect involved single doses (approximately 600 rad) given to mice at different circadian stages. The normal circadian rhythm in cell division was never obliterated. Inhibition of mitosis was evident but unpredictable, ranging from 6 to 15 hr after irradiation. Recovery was evident only during the daily increase in mitotic index of controls. To analyze and review the most common focal lesions of the external auditory canal (EAC), describing the computed tomography (CT) findings. The CT images of EAC were reviewed in a series of 167 patients. Abnormal findings were recorded in 13 cases. The CT images were correlated with otoscopic and/or intraoperative findings. There were four cases of osteoma (one located outside the EAC); all of which were treated surgically, as was the one case of exostosis. Six patients presented debris in EAC, coinciding with clinical evidence of external otitis. There were two cholesteatomas derived from EAC, one of them secondary to survery to treat osteoma. A retrievable nitinol endobronchial stent : an experimental study in dog

The purpose of this study was to evaluate the safety and the retrievability of a new covered retrievable nitinol tracheobronchial stent. Stents were knitted from 0.2mm nitinol wire, covered with polyurethane, and were 20-22mm in diameter and 2cm in length. Under fluoroscopic guidance, a stent was placed in the normal right bronchus intermedius of ten dogs. Using a retrieval hook, stent retrieval was attempted after 1 month(N=5) or 2 months(N=5). After removal, the dogs were sacrificed and their their tracheobronchial trees were examined grossly and histologically. Eleven stents were successfully placed in ten dogs. Migration and expectoration occurred in four of ten stents in nine dogs(40%). Five stents were successfully removed from six dogs(83%). (1-/sup 14/C)-lauric acid (LA) was studied as reference compound. Fifty-two percent (MCDD), 32% (PCDD I), and 8% (PCDD II) of the radioactive doses were exhaled as /sup 14/CO/sub 2/ during 12 h after i.v. injection. Similar results were obtained after p.o. administration. Radiation therapy in the treatment of hilar cholangiocarcinoma

The incidence of hilar cholangiocarcinoma is very rare worldwide. Radical resection is the only prognostic factor for long survival in patients with hilar cholangiocarcinoma. Postoperative radiation therapy can improve local control and survival rates for patients with palliative resection, but it remains controversial in patients with radical resection. Biliary drainage can effectively release bile duct obstruction for the majority of patients with locally advanced disease, and may even prolong survival when combined with radiation therapy. Radiation therapy includes extrernal beam therapy alone, external beam therapy with intraluminal brachytheapy and new radiation technique, such as three dimentional conformal therapy and intensity modulated radiation therapy. Numerical and experimental investigation of shellside characteristics for RODbaffle heat exchanger

RODbaffle heat exchanger is a kind of shell-and-tube heat exchanger with eminent performance. Because of the characteristics of tube bundle support structure, fluid flow on shellside is longitudinal and periodical, and fluid flow is of symmetry in landscape orientation. According to the fluid flow characteristics on shellside and ignoring the impact of inlet, outlet and shell wall, a periodic flow unit duct was taken as the simplified model of the shellside to perform numerical simulation by using CFD code, FLUENT. It was found that the both errors in magnitude of the main fluid velocities and heat transfer coefficients between results of simulation and that of experiment or correlations are less than 10%, and the errors of pressure drop between simulation and correlation are no more than 20%. Various factors should be considered in the surgical planning for curative resection. These factors include extent of bile duct involvement, relationship between portal vein and tumor involvement, diffuse hepato duodenal ligament infiltration, vascular invasion, lymph node metastasis, peritoneal seeding, and hepatic volume. Fundamental study of heat transfer and flow situation around a spacer (in the case of a cylindrical rod as a spacer)

This paper describes the heat transfer augmentation and the flow situation around a single spacer (a cylindrical rod) on the heated surface of a parallel plate duct in order to examine basically the effects of the spacer in the fuel elements of a high temperature gas-cooled reactor. The ends of the cylindrical rod contact the upper and lower planes. A thermosensitive liquid crystal film is used to indicate the effective area for the heat transfer. The mean Nusselt number, which is estimated within the optional distance from the spacer to the downstream direction, peaks at a dimensionless distance of X/D = 1-3, and after that decreases gradually with the flow direction. The manner in which heat transfer corresponds to the flow situation is also examined. The horseshoe vortex, produced around the spacer, affects the wake and contributes to the increase of the local heat transfer. (author).

A major new initiative to develop and irradiate a long-lived, mixed-oxide (MOX) fuel system in the Fast Flux Test Facility (FFTF) has been implemented by Westinghouse Hanford Company for the U.S. Department of Energy. The purpose of this new fuel system, called the core demonstration experiment (CDE), is to demonstrate the capability of achieving a 3-yr life in a prototypical, heterogeneous reactor environment under prototypical power and temperature conditions. Ten fuel and six blanket CDEs are establishing the performance characteristics of entire fuel assemblies of wire-wrapped, large-diameter, annular-pellet, advanced MOX fuel pins with the tempered martensitic HT-9 alloy cladding and end caps, HT-9 wire wrap, and an HT-9 duct in a heterogeneous array with the blanket assemblies. The CDE performance characteristics are confirming the basis for design, fabrication, and irradiation of the CDE.

An investigation of the thermal performance of an oil filled transformer winding

Manufacturers may be required to design transformers to overload specifications. Oil viscosity changes and loss variation with temperature are not accounted for in the industry loading guide. A comprehensive test program was performed using a winding assembly with imbedded thermocouples. Steady state and transient tests were performed for natural and non-directed forced oil circulation. The temperature distribution in the winding was not linear with height as usually assumed. A large number of temperature sensors is required to determine the hot spot location. The location of the hot spot was found to be different and less predictable for natural oil circulation compared with non-directed forced oil circulation especially during overload conditions. This paper reports that improved loading guide equations were developed to account for oil viscosity, resistance change, cooling duct oil rise, and location of the hot spot.

Radiotherapy and multimodality management of cholangiocarcinoma

Purpose: To evaluate the results of radiotherapy in cholangiocarcinoma patients managed with various combinations of chemotherapy and surgical resection with selective liver transplantation. Methods and Materials: From January 1990 to December 1995, 61 patients with histologically confirmed biliary duct adenocarcinoma were seen in the Radiation Oncology Department of the University of Pittsburgh. Median follow-up was 22 months (1 to 91 months). The extent of surgery was complete resection in 23 patients (including 17 with orthotopic liver transplant), partial resection in 4, and biopsy in 34. All patients had radiotherapy; median dose was 49.5 Gy. Thirty patients received chemotherapy: 5-fluorouracil (5-FU)-leucovorin with interferon alpha (IFN#alpha#) in 27, and taxol in 3. To analyze the enhancement patterns and morphology of peripheral cholangiocar-cinomas (CCs), as seen on three-phase helical CT. Three-phase helical CT scans of 25 cases of 24 patients with pathologically-proven CCs were retrospectively reviewed. After intravenous administration of contrast media, scans of the hepatic arterial phase (HAP) were obtained at 30 seconds, of the portal venous phase (PVP) at 70 seconds, and of the delayed phase (DP) at 3 minutes. Linear, dense, marginal enhancement was seen in 17 CCs (68%), mainly on HAP images (64%) while thick, band-like, peripheral enhancement was seen in 18 (72%), on either PVP (52%) or DP (44%). Internal enhancement was observed in 23 CCs (92%) on PVP (68%) and/or DP (88%), and 19 CCs (76%) among them showed incomplete enhancement of stippled and/or septated patterns. Cigarette smoking is the major environmental risk factor for bladder cancer in humans. Aromatic amines, potent DNA-reactive bladder carcinogens present in cigarette smoke, contribute significantly. However, increased cell proliferation, caused by direct mitogenesis or in response to cytotoxicity, may also play a role since urothelial hyperplasia has been observed in human cigarette smokers. We examined the urothelial effects of cigarette smoke (whole body inhalation exposure (Teague) system) in female C57BL/6 mice at various times in two studies, including reversibility evaluations. In both studies, no urothelial hyperplasia was observed by light microscopy in any group. To report a comparative prospective long-term functional analysis after Riboflavin UV A corneal cross-linking (CXL) in three different age groups of patients affected by progressive keratoconus (KC). Methods. Functional analysis comprised paediatric patients (?18 years) included 152 eyes (29.5%); intermediate group (19-26 years) 286 eyes (55.4%), and adults (?27 years) 78 eyes (15.1%). CXL was performed according to the Siena protocol by using the Vega CBM (Caporossi-Baiocchi-Mazzotta) X linker (CSO, Florence, Italy) at Siena University by the same authors. The temperature measurements were made by using both a laser holographic interferometer and thermocouples. In addition, the reattachment length was determined by flow visualization. On-line measurements of nitrogen oxides and total amounts of unburnt hydrocarbons were performed. Hydrogen cyanide, hydrogen chloride and ammonia in the smoke were sampled and absorbed in impinger bottles and subsequently analyzed using wet chemical techniques. Rehabilitation of a gassy low pressure liquid filled 115 kV cable

The recent attempt to degassify a self contained liquid filled (SCLF) cable in Ontario Hydro is described. In September 1989, there was a high pressure alarm on one phase of a SCLF 115 kV cable circuit. The direct buried cable circuit is about 1,890 m long with two joints and was installed in 1968. Oil samples from the cable had 10 to 12% gas content with high levels of hydrogen, methane, and ethane, and very low levels of carbon monoxide, carbon dioxide, and acetylene. From these results it was concluded that the gas was generated by low energy electrical discharges and the paper insulation was not degraded. Various utilities and cable manufacturers were contacted to find if they had experienced similar problems. With a pilot electrostatic precipitator having wider plate spacings than usual tests were performed under industrial conditions. In the two-section two-duct pilot precipitator with a precipitation area of 60 m/sup 2/ the plate spacing was increased up to 1,000 mm. As gas velocities values between 0.5 and 2.0 m/s were selected. The precipitator was tested with the flue gases of a coal-fired power station and in the room dedustion of an iron ore sintering plant. This paper presents a methodology for modeling slug initiation and growth in horizontal ducts. Transient two-fluid equations are solved numerically using a class of high-resolution shock capturing methods. The advantage of this method is that slug formation and growth in a stratified regime can be calculated directly from the solutions to the flow field differential equations. In addition, by using high-resolution shock capturing methods that do not contain numerical diffusion, the discontinuity generated by slugging in the flow field can be modeled with good accuracy. The two-fluid model is shown to be well-posed mathematically only under certain conditions. Heat losses in duct flow and heat transfer enhancement are investigated through an analysis of natural convection about a horizontal cylinder with a porous or fibrous coating. The porous substrate may be used for two purposes. According to its properties, it may be employed as an insulating material or as a means to surface augmentation. An optimization study is then carried out in order to find the best conditions that allow good thermal insulation or heat transfer rate improvement. Intraoperative electron beam therapy and external photon beam therapy with lumpectomy as primary treatment for early breast cancer

Between 1984 and 1988, seven women with breast cancer were treated with intraoperative electron beam therapy (IOEBT). All patients were Caucasian. Patient age ranged from 37 to 62 (median 53) years. Histopathology was infiltrating duct carcinoma (6/7) or medullary carcinoma (1/7). Four tumors were staged T1 N0 MO, one T1 N1 MO, one T2 N1 MO, and one lesion was recurrent. At the time of primary lumpectomy or axillary node dissection, a dose of 10 or 15 Gy IOEBT was administered to the tumor bed with 6 or 9 MeV electrons through the lumpectomy wound. All patients received 45 or 50 Gy over five to six weeks with 6 MV photons to the breast and, in four patients, to the regional nodes. Three patients received adjuvant chemotherapy and tamoxifen. To assess the clinical and pathologic features of each type of intrahepatic cholangiocarcinoma, which is divided into three types according to gross appearance, and to determine the efficacy of CT in detecting this tumor. The pathologic and CT features of 53 surgically proven cases of intrahepatic cholangio-carcinoma were reviewed. On the basis of their gross appearance, the tumors were divided into three types, as follows : mass forming (n=33), periductal infiltrating (n=6), and intraductal growth type (n=14). The experiment involved the use of a scaled model of a commercial swine facility consisting of 2 small chambers connected by a duct containing the filters. A 5 kg naive pig was placed in the reception chamber for a period of 6 hours after aerosolization with porcine reproductive and respiratory syndrome virus (PRRSV). Blood samples from pigs were collected before and after aerosolization to test for the presence of PRRSV RNA. Only blood samples were tested for PRRSV antibodies by IDEXX 2XR ELISA. None of the 9 pigs tested were found to be infected. Development of HT-9 for liquid-metal reactor components

Alloy HT-9 is being used for both duct and cladding applications in advanced liquid-metal reactor (LMR) experiments. This tempered martensitic steel was selected for use as an LMR core component material primarily because of its excellent resistance to radiation-induced swelling. Experiments conducted in the Fast Flux Test Facility (FFTF) at 410 degree C and exposures in the range of 150 to 175 displacements per atom (dpa) have shown that Ht-9 exhibits only a 0.2 to 0.3% increase in volume. Cold-worked austenitic steels exhibit volumetric increases of 20 to 30% at 410 degree C, Alloy HT-9 is being used for a series of fuel pin experiments in the FFTF, and these tests have achieved a burnup of 175 MWd/kg metal and a fluence of 25 x 10"2"2 n/cm"2 (E > 0.1 MeV) without fuel pin breach. The high confidence placed in HT-9 is based on a wide series of in- and ex-reactor experiments. Test results for these experiments are summarized in this paper.

This report describes the combustion experiments performed using the SP Industry Calorimeter. In addition to measuring parameters such as heat release rate (HRR), mass loss rate, effective heat of combustion and smoke obscuration, considerable effort was put into the characterisation of the smoke gases in order to obtain a basis for evaluations of the toxic potency of the smoke. The materials studied were Nylon 66, polypropene, polystyrene (with and without fire retardant, respectively), PVC and chlorobenzene. A total of 19 large-scale, well ventilated combustions were carried out. The mass of sample burned ranged from 20 kg to 125 kg in an experiment. A procedure was designed for cleaning the smoke gas duct between experiments and was found to be effective. Peritonitis is the major problem, but the continuation of treatment may also be undermined by different complications, such as peritoneal leakage, hernia, catheter malfunctioning, and scrotal-penile edema; a careful investigation of the patient is always needed in such cases. From November 1985 to February 1990, we examined 20 patients, who had presented with different types of complications in the course of dialysis. Sonographic and CT findings of six patients with cholangiocarcinoma of the liver associated with recurrent pyogenic cholangitis are presented. The cases were among 128 patients who underwent partial resection of the liver because of recurrent pyogenic cholangitis and its complication. An approximate expression for whistler-mode group velocity is obtained, taking into account the effects of electron temperature and anisotropy, density and ion effects, effects of oblique propagation and a non-dipolarity of the dayside magnetospheric magnetic field. This expression is applied to the propagation of whistlers between one hemisphere and the other. It is pointed out that at frequencies close to the upper cut-off of whistler spectra, perturbations to whistler group delay times due to temperature effects can be of the same order of magnitude as, or even higher than, the corresponding perturbations due to finite electron density and ion effects. Full text: Lung cancer is responsible for most of the cancer related deaths and calls for new approaches to control the menace. In the present study chemopreventive efficacy of curcumin and quercetin was investigated against benzo(a)pyrene (BP) induced lung carcinogenesis. The mice were segregated into five groups which included normal control, BP treated, BP+curcumin treated, BP+quercetin treated and BP+curcumin+quercetin treated groups. The morphological and histological analyses of tumor nodules confirmed lung carcinogenesis, after 22 weeks of single i.p. injection of BP at a dose of 100 mg/Kg body weight to mice. Tumor incidence and tumor multiplicity were observed to be 88% and 1.75, respectively in the BP treated mice. We wanted to determine the technical and clinical efficacy of using a PTFE-covered self-expandable nitinol stent for the palliative treatment of malignant biliary obstruction. Thirty-seven patients with common bile duct strictures caused by malignant disease were treated by placing a total of 37 nitinol PTFE stents. These stents were covered with PTFE with the exception of the last 5 mm at each end; the stent had an unconstrained diameter of 10 mm and a total length of 50 80 mm. The patient survival rate and stent patency rate were calculated by performing Kaplan-Meier survival analysis. The bilirubin, serum amylase and lipase levels before and after stent placement were measured and then compared using a Wilcoxon signed-rank test. To investigate the problems of the Hanaro stent (Solco Intermed, Seoul, Korea) when used in the palliative treatment of patients with inoperable malignant biliary obstruction. Between January 2000 and May 2001, the treatment of 46 patients with malignant biliary obstruction involved percutaneous placement of the Hanaro stent. Five patients encountered problems during removal of the stent's introduction system. Since the very high temperature reactor (VHTR) components such as hot gas ducts and intermediate heat exchanger (IHX) are operated during a design life of 30 to 60 years at 950 .deg. C and 3-8MPa in He impurities, their components are required to have good high temperature strength, creep-rupture strength, high temperature stability, and good corrosion resistance. Among them, creep properties are the most important, because the integrity of their components should be preserved during a design life of over 30 years at the maximum operating temperature up to 1000 .deg. C. So far, there are no materials approved by ASME III which is a nuclear code for temperatures reaching 1000 .deg. C, and until now, a material selection is still left as a key issue. Introducing DeBRa: a detailed breast model for radiological studies

Currently, x-ray mammography is the method of choice in breast cancer screening programmes. As the mammography technology moves from 2D imaging modalities to 3D, conventional computational phantoms do not have sufficient detail to support the studies of these advanced imaging systems. Studies of these 3D imaging systems call for a realistic and sophisticated computational model of the breast. DeBRa (Detailed Breast model for Radiological studies) is the most advanced, detailed, 3D computational model of the breast developed recently for breast imaging studies. A DeBRa phantom can be constructed to model a compressed breast, as in film/screen, digital mammography and digital breast tomosynthesis studies, or a non-compressed breast as in positron emission mammography and breast CT studies. Expression of Na(+)-H+ exchange and ATP-dependent proton extrusion in growing rat IMCD cells

As the last step of urinary acidification, the inner medullary collecting duct (IMCD) is thought to secrete protons into the tubular lumens by means of a H(+)-translocating adenosinetriphosphatase (H(+)-ATPase). However, recent studies have also shown the existence of Na(+)-H+ exchange activity in IMCD cells. Although the physiological function of the antiporter in IMCD cells is unknown, activation of Na(+)-H+ exchange in other cell-culture systems has been suggested to be closely associated with the process of cell growth. Thus presence of Na(+)-H+ exchange may relate to the growth phase of these cells. Experiences in radioactive gaseous effluent management in JAERI

In the Japan Research Reactor-II (JRR-2), the main source of _4_1Ar generation is the exhaust air from the horizontal experimental holes and the pneumatic tubes. For the horizontal experimental holes, the flow of exhaust air through the holes was decreased by improving the airtightness, and a decay duct of capacity 2.4 m_3 was installed in the middle of the exhaust line. In consequence, the release rate of _4_1Ar was reduced by 6-8%. For the pneumatic tubes, a mechanical shutter was installed in the tube. The shutter stops the exhaust air flow, except when the pneumatic tube is used. Prior to the use, the activated air in the tube is led to a decay tank. As a result, the _4_1Ar release rate was reduced by 10-20%. By the above means, the yearly exposure at the site boundary was reduced to 0.36 mR from 2.6 mR. Estimated costs of ventilation systems complying with the HUD ventilation standard for manufactured homes

At the request of the US Department of Housing and Urban Development (HUD), the Pacific Northwest Laboratory estimated the material, labor, and operating costs for ventilation equipment needed for compliance with HUD`s proposed revision to the ventilation standard for manufactured housing. This was intended to bound the financial impacts of the ventilation standard revision. Researchers evaluated five possible prototype ventilation systems that met the proposed ventilation requirements. Of those five, two systems were determined to be the most likely used by housing manufacturers: System 1 combines a fresh air duct with the existing central forced-air system to supply and circulate fresh air to conditioned spaces. System 2 uses a separate exhaust fan to remove air from the manufactured home. The estimated material and labor costs for these two systems range from $200 to $300 per home. The purpose of this study was to assess the energy flux and energetic balance related to the swine production and the recycling of residues generated by the system for utilization as organic fertilizers. The experiment was carried out at Vale dos Ipes Farm, located in the city of Ouro Verde do Oeste, in the Western of Parana State. One finishing phase swine unity containing 600 animals was monitored from January to June 2005. The treatment system is composed by one steel digester with capacity for 50 m{sup 3}, one sedimentation tank, one algae tank and one bio fertilizer storage tank. The swine barn cleaning is performed by dry scratching on a daily basis. The generated residues flow by gravitation through ducts towards the digester. The duration of the hydraulic retention period was 12 days. The interfacial area transport equation dynamically models the two-phase flow regime transitions and predicts continuous change of the interfacial area concentration along the flow field. Hence, when employed in the numerical thermal-hydraulic system analysis codes, it eliminates artificial bifurcations stemming from the use of the static flow regime transition criteria. Accounting for the substantial differences in the transport phenomena of various sizes of bubbles, the two-group interfacial area transport equations have been developed. The group 1 equation describes the transport of small-dispersed bubbles that are either distorted or spherical in shapes, and the group 2 equation describes the transport of large cap, slug or churn-turbulent bubbles. Accurate diagnosis of local invasion of hilar cholangiocarcinomas is challenging due to their small size and the anatomic complexity of the hepatic hilar region. On the other hand, the correct diagnosis of local invasion is essential for assuring the possibility of curative surgery. The purpose of this study was to evaluate the feasibility of four-channel multidetector-row computed tomography (MDCT) in the assessment of vascular and bile duct involvement, by which we could obtain useful information for the surgical management of hilar cholangiocarcinoma. The subjects were 18 patients for whom the extent of tumor invasion was surgically and pathologically confirmed. All patients underwent preoperative multiphasic CT scanning by MDCT. Due to its inherent shortcomings, however, such static correlations are inaccurate and present serious problems in the numerical analysis. In view of this, a new dynamic approach employing the interfacial area transport equation has been studied. For heavy-ion beam driven inertial fusion ''liquid-protected'' reactor designs such as HYLIFE-II, a mixture of molten salts made of F{sup 10}, Li{sup -6}, Li{sup 7} and Be{sup 9} (called flibe) allows small chambers and final-focus magnets closer to the target with superconducting coils suffering higher radiation damage, though they can stand only a certain amount of energy deposited before quenching. This work has been primarily focusing on verifying that total energy deposited by fusion neutrons and induced gamma rays remain under such limit values and the final purpose is the optimization of the shielding of the magnetic lens system from the points of view of the geometrical configuration and of the physical nature of the materials adopted.
