Today's doctors need to be skilled communicators who know how to identify, prevent and manage adverse events and near misses, how to use evidence and information, how to work safely in a team, how to practise ethically, and how to be workplace teachers and learners. These competencies (knowledge, skills and attitudes) are set out in the National Patient Safety Education Framework (NPSF) of the Australian Council for Safety and Quality in Health Care. In consistency with the increasing requests from Member States for establishing radiotherapy programmes, an IAEA Technical Co-operation project was initiated in Latin America aimed at improving the physical aspects (as a complement to the clinical issues) of quality assurance in radiotherapy; this ARCAL XXX project (RLA/6/032) was classified as a Model Project of the IAEA. Among the important outcomes of the project were (i) the training of a considerable number of medical physicists in hospitals of the region, (ii) the development of a protocol for quality control procedures, and (iii) the organization of quality audit site visits (to the participant countries) where the implementation in hospitals of the developed quality control procedures is verified. Although Mexico has made tremendous progress toward achieving universal basic education, school quality has not kept pace with enrollment increases, especially in rural areas. Innovations at the federal and state levels to raise the quality of basic education in Mexico include decentralizing the education system, thereby giving the states more control over education, and creating the Carrera Magisterial, a program of incentives and professional development for teachers. The first part of this paper shows that early in their professional lives, teachers in basic public schools are better paid than other comparable groups. The natural gas industry has proposed an increase in the DOE gas R&D budget from about $100 million to about $250 million per year for each of the next 10 years. The proposal includes four programs: natural gas supplies, fuel cells, natural gas vehicles and stationary combustion systems. This paper is a qualitative assessment of the gas industry proposal and recommends a natural gas R&D strategy for the DOE. The methodology is a conceptual framework based on an analysis of market failures and the energy policy objectives of the DOE`s (1991) National Energy Strategy. This framework would assist the DOE in constructing an R&D portfolio that achieves energy policy objectives. The first stage, covering Poland as a whole, was recently completed, jointly financed with the U.S. E.P.A. and the Government of the Netherlands. The second stage, now under way, will extend the analysis into a specific regional framework. Stage 2 is financed by the World Bank, the Polish Government, and the U.K. Environment Know-How Fund. Air Pollution in Poland is overwhelmingly caused by energy production and uses, due to the fact that the Polish economy depends heavily on coal and lignite. The HIPAA privacy rule (HIPAA) has had both positive and negative effects on the release of patient information by healthcare facilities. Although the intention of HIPAA was to protect patient privacy and to promote security and confidentiality of patient information, it has had unintended consequences for facilities. To identify some of these unintended effects, two expert panels of health information management directors from healthcare facilities participated in the nominal group technique meetings. They identified 70 barriers related to release of patient information associated with the implementation of HIPAA. The current state and need for radiation protection training in Finland have been surveyed by the Radiation and Nuclear Safety Authority STUK. The survey sought to determine whether the current requirements for radiation protection training had been met, and to promote radiation protection training. Details of the scope and quality of present radiation protection training were requested from all educational institutes and organizations providing radiation protection training. The survey covered both basic and further training, special training of radiation safety officers, and supplementary training. The questionnaire was sent to 77 educational organization units, 66 per cent of which responded. Radiation workers and radiation safety officers were asked about radiation protection knowledge and needs for additional training. This Final Environmental Impact Statement and Environmental Impact Report (Final EIS/EIR) has been prepared to meet the requirements of the National Environmental Policy Act (NEPA) and the California Environmental Quality Act (CEQA). The Proposed Action includes the construction, operation, and decommissioning of a 48 megawatt (gross) geothermal power plant with ancillary facilities (10-12 production well pads and 3-5 injection well pads, production and injection pipelines), access roads, and a 230-kilovolt (kV) transmission line in the Modoc National Forest in Siskiyou County, California. Alternative locations for the power plant site within a reasonable distance of the middle of the wellfield were determined to be technically feasible. Three power plant site alternatives are evaluated in the Final EIS/EIR.

In August 1995, the US Nuclear Regulatory Commission (NRC) issued a policy statement proposing improved regulatory decision making by increasing the use of probabilistic risk assessment (PRA) in all regulatory matters to the extent supported by the state-of-the-art in PRA methods and data. A key aspect in using PRA in risk-informed regulatory activities is establishing the appropriate scope and attributes of the PRA. In this regard, the American Society of Mechanical Engineers (ASME) decided to develop a consensus PRA standard. The objective is to develop a PRA standard such that the technical quality of nuclear plant PRAs will be sufficient to support risk-informed regulatory applications. The authors present example recommendations for the systems analysis element of a PRA for incorporation into the ASME PRA standard.

Two-tier networks, comprising a conventional cellular network overlaid with shorter range hotspots (e.g. femtocells, distributed antennas, or wired relays), offer an economically viable way to improve cellular system capacity. The capacity-limiting factor in such networks is interference. The cross-tier interference between macrocells and femtocells can suffocate the capacity due to the near-far problem, so in practice hotspots should use a different frequency channel than the potentially nearby high-power macrocell users. Centralized or coordinated frequency planning, which is difficult and inefficient even in conventional cellular networks, is all but impossible in a two-tier network. In the late 1970's, the Department of Energy recognized that there were significant safety and cost advantages associated with immobilizing the high-level waste in a stable solid form. Several alternative waste forms were evaluated in terms of product quality and reliability of fabrication. This evaluation led to a decision to build the Defense Waste Processing Facility (DWPF) at SRS to convert the easily dispersed liquid waste to borosilicate glass. Health information technology initiatives created the framework for a national health information infrastructure that concomitantly fostered a need to build intellectual capacity within our current and future health information management (HIM) work force. Results from the 2008 HIM Educator Survey are discussed. Developed for voluntary electronic participation, the survey comprised a series of questions about educators' professional interests and responsibilities. Summary data from the 402 respondents are provided and highlight areas such as academic rank, teaching status, salary range, levels of interest in various issues, and use of virtual learning tools. Data from this survey provide insights into the concerns and challenges many HIM educators face in today's training institutions and suggest implications for future directions in work force training and professional development within the HIM field. Research is needed to best define the optimal use of ICU telemedicine, but efforts are hindered by methodological challenges and the lack of an organized delivery approach. We convened an interdisciplinary working group to develop a research agenda in ICU telemedicine, addressing both methodological and knowledge gaps in the field. To best inform clinical decision-making and health policy, future research should be organized around a conceptual framework that enables consistent descriptions of both the study setting and the telemedicine intervention. The framework should include standardized methods for assessing the preimplementation ICU environment and describing the telemedicine program. The HARVEST process chosen is an adaptation of the FINGAL concept developed at Harwell 15 years ago. Because of certain changes in engineering design and differences in the composition of the waste, additional chemical engineering data are required for HARVEST. The new inactive pilot plant has been built to provide this information. Its size is virtually full-scale and batches of glass up to 1 tonne can be manufactured in vessels of diameter up to 750mm. Five glass-making runs have been carried out. Attention has concentrated on the performance of the off-gas system which employs wet methods. I formulate a model in which money coexists with equity shares on a risky aggregate endowment. Agents can use equity as a means of payment, so shocks to equity prices translate into aggregate liquidity shocks that disrupt the mechanism of exchange. I characterize a family of optimal monetary policies and find that the resulting equity prices are independent of monetary considerations. I also study a perturbation of the family of optimal policies that targets a positive constant nominal interest rate and find that in this case the real equity return includes a liquidity return that depends on monetary considerations.

Pyrethroid pesticides occur in urban creek sediments at concentrations acutely toxic to sensitive aquatic life. To better understand the source of these residues, runoff from residential neighborhoods around Sacramento, California was monitored over the course of a year. Pyrethroids were present in every sample. Bifenthrin, found at up to 73 ng/L in the water and 1211 ng/g on suspended sediment, was the pyrethroid of greatest toxicological concern, with cypermethrin and cyfluthrin of secondary concern. The bifenthrin could have originated either from use by consumers or professional pest controllers, though the seasonal pattern of discharge from the drain was more consistent with professional use as the dominant source. Stormwater runoff was more important than dry season irrigation runoff in transporting pyrethroids to urban creeks. The role of law in a federal regulatory agency is examined from an organizational perspective. While law is usually viewed in terms of its legal, political, and social value consequences, it is postulated that it also has significant organizational consequences. The impact of those consequences is examined in the case of a single agency, the US Nuclear Regulatory Commission. The legal process is shown to be a powerful organizational characteristic of the government administrative agency, beginning with statuatory definition of organization goals, structure, and procedures and ending with judicial review of actions. Agency lawyers are shown to represent a distinct professional subculture within this agency. Their values and orientations toward business, the role of regulation, and the role of nuclear utilities are different from the 95% of agency employees subculture based on a physical science/engineering background. To illustrate the implementation of a bottom-up approach to the study of culture in health disparities, this article describes the development of a cultural cancer screening scale (CCSS) using mixed methodologies. The aim was to identify cultural factors relevant to breast and cervical cancer screening, develop an instrument to assess them and examine its preliminary psychometric properties among Latin American (Latino) and non-Latino White (Anglo) women in Southern California. Seventy-eight Latino and Anglo women participated in semi-structured interviews, which were content coded based on Triandis' methods for the analysis of subjective culture. Based on the emerging cultural elements, items relevant to cancer screening were developed and pilot tested with 161 participants. Objective was to follow the cognitive and creative processes demonstrated by student research participants as they integrated a developing knowledge of ``big`` science, as practiced at LLNL, into a personal and idiosyncratic visual, graphical, or multimedia product. The participants, all non-scientists, involved in this process, attended a series of design classes, sponsored by LLNL at the Art Center College of Design in Pasadena CA. As a result of this study, we have become interested in the possibility of similar characteristics between scientists and artists. We have also become interested in the different processes that can be used to teach science to non-scientists, so that they are able to understand and portray scientific information.

In recent years, several groups, including the physician assistant, health information management, ophthalmic medical technician/technologist, and athletic training review committees, have left the Commission on Accreditation of Allied Health Education Programs (CAAHEP) system to form their own, profession-specific accrediting bodies. Their motivation was typically a desire for greater professional visibility and autonomy. Combined, these professions represented one third of the CAAHEP's programs. This article reviews the history of allied health and examines current attempts to bring cohesion and identity to this increasingly fractured segment of the U.S. health care system. PMID:18444439

In an attempt to develop and prioritize health and wellness programs, this study investigates the factors significantly affecting the health conditions of professional bus drivers, as well as the strength of these factors. This study uses self-rated health as the examination measurement. This simple assessment is an inclusive measure of health status for judging health trajectory, and is highly associated with changes in functional ability, including perceived control over driving. This article examines relevant government policy documents on education and culture and discovers that Nigerian education authorities do not "discriminate" against art and culture in its articulation of educational policies per se, but lack of administrative machinery or political will has resulted in the deprivation of the Nigerian child in the process of creative activity in early childhood development. The article argues that lack of creative ability in our educational products is perhaps a major setback in the nation's quest for industrial and technological development. Full text.The electric energy demand in Lebanon is estimated to grow at an average of 3-5% per year for the coming 10 years. Such an increase in energy demand is problematic for Lebanon since its economy is almost totally dependent on imported fuel which contributes to 97% of the overall energy requirements. Solar water heaters (SWH) are regarded as the most important element in a long term energy conservation and management strategy for this country, but their promotion is a national issue requiring the participation of many stake holders and decision makers. Additionally, the success of solar energy penetration into the existing energy market is constrained by many factors such as technical and financial limitations, decision criteria and policy instruments. This paper examines the global impacts of a policy that internalizes the external costs (related to air pollution damage, excluding climate costs) of electricity generation using a combined energy systems and macroeconomic model. Starting point are estimates of the monetary damage costs for SO{sub 2}, NO {sub X} , and PM per kWh electricity generated, taking into account the fuel type, sulfur content, removal technology, generation efficiency, and population density. Internalizing these externalities implies that clean and advanced technologies increase their share in global electricity production. Particularly, advanced coal power plants, natural gas combined cycles, natural gas fuel cells, wind and biomass technologies gain significant market shares at the expense of traditional coal- and gas-fired plants. This paper addresses the problem of fair equilibrium selection in graphical games. Our approach is based on the data structure called the {\\em best response policy}, which was proposed by Kearns et al. \\cite{kls} as a way to represent all Nash equilibria of a graphical game. In \\cite{egg}, it was shown that the best response policy has polynomial size as long as the underlying graph is a path. In this paper, we show that if the underlying graph is a bounded-degree tree and the best response policy has polynomial size then there is an efficient algorithm which constructs a Nash equilibrium that guarantees certain payoffs to all participants. Another attractive solution concept is a Nash equilibrium that maximizes the social welfare. This paper develops a model of nonlinear pricing with competition. The novel element is that each consumer's willingness to pay for quality is private information and is allowed to differ across brands. The consumer's preferences are represented by a multidimensional type containing the marginal value of quality for different products. Buyers with high willingness to pay for quality also display strong preferences for particular brands, and require higher discounts in order to switch away from their favorite product. This paper presents a framework for creating a common spatial canvass that can bring together considerations of resource availability, infrastructure reliability, and development costs while strategizing renewable energy investment. We describe the underlying models and methodologies that annotate an investment plan for potential sites over a time-period with costs and constraints which may be imposed on distance from infrastructure, system impact on infrastructure, and policy incentives. The framework is intended as an enabler for visualization, optimization and decision making across diverse dimensions while searching for lucrative investment-plans.

High-voltage transmission lines can pose an environmental problem, especially in countries where the electricity production is at a distance from the demand location. Planning, construction, and operation of high voltage transmission lines should be conducted within a comprehensive environmental policy, which may include assessment of visual impact, landscape and wildlife protection, and reduction of health risks. This paper reviews the environmental policies and action in planning, construction, and operation as developed by the utilities members of the Union Internationale des Producteurs et Distributeurs d`Energie Electrique (UNIPEDE). The paper also presents a brief outline of the health issues associated with the effects of electromagnetic fields to humans.
