The interventional treatment for biliary recurrent obstruction after palliative T tube drainage in patients with obstruction due to cholangiocarcinoma

The purpose of this study was to evaluate the safety and efficacy of TACE with microspheres preloaded with doxorubicin in unresectable intrahepatic cholangiocarcinoma (UCH). Twenty patients with UCH were observed; 9 refused, preferring other palliative care or chemotherapy, and 11 agreed to be treated with one or more cycles of DC beads loaded with doxorubicin (100-150 mg) in a TACE procedure between February 2006 and September 2007. A total of 29 individual TACE procedures were performed. Follow-up imaging was performed on all patients before, immediately after, and 4 weeks after each TACE procedure to evaluate the response and need for further treatment. Each patient received i.v hydration, antibiotics, and medications against nausea and pain before TACE. Survival rate was calculated using Kaplan-Meier survival curve. Percutaneous Treatment of Malignant Jaundice Due to Extrahepatic Cholangiocarcinoma: Covered Viabil Stent Versus Uncovered Wallstents

Objective: To explore the interventional method to treat biliary recurrent jaundice after T tube drainage in patients with malignant obstructive jaundice due to cholangiocarcinoma. Methods: 7 biliary metallic stents were placed in 7 patients with recurrent jaundice after T-tube drainage in cholangiocarcinoma cases. Results: Stent placement was once successful in all 7 cases with successful rate of 100%. For all cases, TBIL, ALT, GTP and AKP values 7 days postoperatively were significantly lower than that of preoperation together with subsidence of jaundice satisfactorily for 100% after the treatment. Temporary endovascular control of a bleeding aortoenteric fistula by transcatheter coil embolization.

To compare clinical effectiveness of Viabil-covered stents versus uncovered metallic Wallstents, for palliation of malignant jaundice due to extrahepatic cholangiocarcinoma, 60 patients were enrolled in a prospective and randomized study. In half of the patients a bare Wallstent was used, and in the other half a Viabil biliary stent. Patients were followed up until death. Primary patency, survival, complication rates, and mean cost were calculated in both groups. Stent dysfunction occurred in 9 (30%) patients in the bare stent group after a mean period of 133.1 days and in 4 (13.3%) patients in the covered stent group after a mean of 179.5 days. Palliation of radiation-related mucositis

A 67-year-old man presented with gastrointestinal bleeding resulting from a fistula between the aortic stump and the fourth part of the duodenum 14 months after an axillobifemoral bypass, graft excision, and aortic stump closure for aortic graft infection. The patient initially refused any attempt at definitive surgical treatment so coil embolization of the aortic stump was performed via a brachial approach. Coil embolization may be a useful adjunct in the management of such difficult problems. This approach may buy time and allow careful planning of a definitive treatment or may serve as a palliative procedure in inoperable patients. PMID:15947052

A Dual Expandable Nitinol Stent: The Long-term Results in Patients with Malignant Gastroduodenal Strictures

Oral mucositis associated with head and neck radiotherapy can substantially hinder completion of cancer therapy. Alleviation of this often severe stomatitis can provide enhanced patient comfort and facilitate appropriate care. A double-blind format was used in a pilot project to measure, against a control rinse, the effectiveness of an oral rinse consisting of hydrocortisone, nystatin, tetracycline, and diphenhydramine in controlling radiation-related mucositis. A combination of clinical evaluation and patient responses to a questionnaire was used to judge the results of the topical medications. Patients using the experimental medication developed less mucositis than did patients in the control group.

Malignant duodenal obstructions: palliative treatment with covered expandable nitinol stent

We wanted to evaluate the long-term results of a dual expandable nitinol stent for the palliative treatment of malignant gastroduodenal strictures. The dual stent consists of two stents; an outer partially nylon covered stent and an inner bare nitinol stent. The outer stent was placed into the stricture and this was followed by coaxial placement of the inner bare stent. Using fluoroscopic guidance, dual expandable stents were placed in 86 patients with inoperable malignant gastroduodenal strictures. The technical and clinical success, the complication, survival and the stent patency were evaluated during the follow-up period. Stent placement was technically successful in 83 of the 86 patients. To evaluate the feasibility and clinical effectiveness of using a polyurethane-covered expandable nitinol stent in the palliative treatment of malignant duodenal obstruction. Under fluoroscopic guidance, a polyurethane-covered expandable nitinol stent was placed in 12 consecutive patients with malignant duodenal obstructions. We wanted to determine the technical and clinical efficacy of using a PTFE-covered self-expandable nitinol stent for the palliative treatment of malignant biliary obstruction. Thirty-seven patients with common bile duct strictures caused by malignant disease were treated by placing a total of 37 nitinol PTFE stents. These stents were covered with PTFE with the exception of the last 5 mm at each end; the stent had an unconstrained diameter of 10 mm and a total length of 50 80 mm. The patient survival rate and stent patency rate were calculated by performing Kaplan-Meier survival analysis. The interventional treatment for biliary re-stenosis after metallic stents placement in patients with malignant obstruction due to cholangiocarcinoma

We wanted to evaluate the usefulness of a new type of a complex expandable nitinol stent that was designed to reduce the stent's propensity to migration during the treatment of malignant gastroduodenal obstructions. Two types of expandable nitinol stent were constructed by weaving a single thread of 0.2mm nitinol wire in a tubular configuration: an uncovered stent 18mm in diameter and a covered stent 16mm in diameter. Both ends of the covered stent were fabricated by coaxially inserting the covered stent into the tubular uncovered stent and then attaching the two stents together with using nylon monofilament. Objective: To explore the interventional treatment for biliary re-stenosis after metallic stents placement in patients due to cholangiocarcinoma and evaluate its therapeutic effect. Twenty-six patients, (15 palliative cases and 11 preoperative) underwent treatment for the relief of colorectal obstruction using metallic stents positioned under fluoroscopic guidance. Transcatheter stent implantation to treat aortic coarctation in infancy.

To investigate the problems of the Hanaro stent (Solco Intermed, Seoul, Korea) when used in the palliative treatment of patients with inoperable malignant biliary obstruction. Between January 2000 and May 2001, the treatment of 46 patients with malignant biliary obstruction involved percutaneous placement of the Hanaro stent. Five patients encountered problems during removal of the stent's introduction system. The RGA reading was taken and it was found out that residual gas inside tube was sulphur hexafluoride. A leak was detected in second tube of unit number eight in between electrode 6 to 8. Leak was sealed with the sealant. Again leak check was done and no leak was found. The tank was closed and conditioning was started again. During the same unit number eight conditioning, leak developed again followed by a spark. So the damaged tube was replaced with a new accelerator tube. During the installation time the alignment of the machine was taken care. Again leak checking was done and the tube was baked properly. The tank was closed again and this particular unit was conditioned for about four days. The maximum voltage it has attained was 1.1 MV. (author)

Depression among Alumni of Foster Care: Decreasing Rates through Improvement of Experiences in Care

Effect of acidic fluoride solution on beta titanium alloy wire.

The Northwest Foster Care Alumni Study examined the relation between experiences in foster care and depression among young adults who spent at least a year in foster care as adolescents. Results indicate that preparation for leaving foster care, nurturing supports from the foster family, school stability, access to tutoring, access to therapeutic service and support, and a shorter and more stable placement history were associated with a lower probability of depression in young adulthood. Results provide evidence that suggest ways to improve practice to decrease rates of depression among alumni of care. (Contains 2 tables.)

Anemia and growth status in pediatric patients receiving maintenance dialysis after a failed renal transplant course: an NAPRTCS report.

The interaction between acidic fluoride solution and beta titanium alloy was investigated to explore the changes that occur in beta titanium alloy by fluoride-containing acetic acid solutions. For this, alloy crystal structure, tensile strength, and elements released from the alloy wires were determined using four solutions (0.05%/pH 6, 0.05%/pH 4, 0.2%/pH 6, and 0.2%/pH 4) for 1 or 3 days. The immersed wire did not form any identifiable new crystal structure compared with the as-received wire. The tensile strength of the immersed wires was significantly reduced compared to the as-received wires in the test solutions if the period of immersion increased from as-received to 3 days. The fractured area of the immersed wire was reduced compared to the as-received one. Care programmes and integrated care pathways

We conducted a retrospective review of the North American Renal Transplant Cooperative Study (NAPRTCS) Registry transplant and dialysis arms to assess anemia and growth patterns in children who returned to dialysis after a failed renal transplant from January 1, 1992 to February 3, 2004. Of the 1807 potential study subjects, 1451 had transplant removal data (TxIn vs. TxOut) available for analysis. Four hundred and twenty-one of 1451 patients (29%) had a transplant nephrectomy at the time of entry into the NAPRTCS Registry Dialysis arm. Anemia rates steadily decreased from 72.2% at 30 days after dialysis initiation to 59.5% at 12 months after dialysis initiation. Factors associated with anemia at 30 days after dialysis initiation included hemodialysis, lack of Epo use, and patients who comprised earlier study era cohorts. A 1,200 bird solar chicken brooder was indigenously designed and operated by the Indian scientists for the first time in the country as a Project under funding by the Ministry of Non Conventional Energy Sources to the All India Women`s Conference. This multi disciplinary project was taken up on the International Sun Day, May 3, 1993 and completed on May, 1994. Data has been collected for the first nine months of operation. GAO's review of the H-Coal and Exxon projects showed inadequacies in DOE's contracting practices and a failure by DOE to properly plan, manage, and monitor, especially the H-Coal project. The initial Government-industry H-Coal agreements regarding the level of investment by private sponsors and the ceilings imposed on sharing in cost growth were imbalanced. Larger investments by private sponsors and sharing of cost growth provides an incentive to private sponsors to control costs and helps to assure that each party is fully committed to the success of the project. The incidence of hepatocellular carcinoma (HCC) is increasing in developed countries and the general status of HCC patients is now by far better than a few years ago, allowing physicians to propose some different treatments of these patients. Among these treatments, intra-arterial injection of 131-iodine-labelled Lipiodol could be used in palliative, curative or adjuvant settings. After a brief summary of the modalities of this treatment, requiring the collaboration of Gastro-enterologists, Oncologists, Radiologists and Nuclear Medicine physicians, the different possibilities of therapeutic uses of this new approach are described and an outlook of these developments is proposed. Palliation of patients with breast carcinoma en cuirasse is a difficult clinical problem which must be effectively managed over extended periods due to the indolent nature of this disease. Self-expanding metallic stent insertion has been widely applied for the palliative treatment of malignant esophageal strictures. Although it is known as an easy, safe, and effective procedure, complications are well known and include things such as stent migration and esophageal stent occlusion caused by tumor in growth. However, metallic stent fractures have been rarely reported in the esophagus, especially for nitinol stents. Palliative hepatic intraarterial chemotherapy (HIC) using a novel combination of gemcitabine and mitomycin C: results in hepatic metastases

The aim of this retrospective study was to determine the safety and efficacy of chemoembolization (TACE) as palliative treatment for patients with unresectable intrahepatic cholangiocarcinoma (CCA) and to compare the results with those in the literature. Fifteen patients with histology-proven CCA (5 men, 10 women) had received palliative treatment with TACE over a 6-year period. The treatment protocol comprised repeated TACE at a minimum of 8-week intervals. TACE was performed with a mixture of 10 ml Lipiodol and 10 mg mitomycin C injected into the tumor-supplying vessels. Follow-up investigations after 8-10 weeks comprised contrast-enhanced multislice spiral CT and laboratory control. Statistical evaluation included survival analysis using the Kaplan-Meier method. To evaluate repeated hepatic intraarterial chemotherapy (HIC) as a palliative treatment option for unresectable cholangiocarcinoma and liver metastases of various origins that were progressive under systemic chemotherapy. Between 2002 and 2006, 55 patients were treated in 4-week intervals (mean five sessions). Combined gemcitabine/mitomycin was administered intraarterially within 1 h. Repeated exposure to cold air reduces the metabolic response to cold air exposure in man. The purpose of this investigation was to measure the change in resting metabolic rate (RMR) with exposure to 22C air and 4C air during a 12 day period. Four women sat in 22C air for 45 min followed by 45 min in 4C air each day for ten days. The authors measured RMR during a 45 min period in 22C air followed by 45 min in 4C air on four days. During the test of the LEDA injector on the CRITS RFQ, an automatic data acquisition system has been implemented. The purpose was to measure the sparking rate of this CW RFQ. The RF level has some influences on vacuum, but there is no evidence of any reciprocal effect. The raw sparking rate is very difficult to interpret, since burst of sparks bias the statistics. A more convenient and useful interpretation is the number of sparking seconds. At the nominal field level (1.75 Kilp), the sparking-second rate is 0.5 per minute without beam. It strongly depends on the field, with a logarithmic law: 4.5 decade/Kilp. With beam, the sparking rate jumps to 3.0 per minute. As far as tested, it depends neither on the beam current (20 to 80 mA) nor on the field (1.5 to 1.7 Kilp tested). With sparking rates as measured here, one could not hope to build an RFQ that would be free of sparks over a several months continuous operation. Image-guided percutaneous cryotherapy for the management of gynecologic cancer metastases

The incidence of hilar cholangiocarcinoma is very rare worldwide. Radical resection is the only prognostic factor for long survival in patients with hilar cholangiocarcinoma. Postoperative radiation therapy can improve local control and survival rates for patients with palliative resection, but it remains controversial in patients with radical resection. Biliary drainage can effectively release bile duct obstruction for the majority of patients with locally advanced disease, and may even prolong survival when combined with radiation therapy. Radiation therapy includes extrernal beam therapy alone, external beam therapy with intraluminal brachytheapy and new radiation technique, such as three dimentional conformal therapy and intensity modulated radiation therapy. The propective randomized clinical study is needed for further investigation in the role of combined modality therapy especially for hilar cholangiocarcinoma. (authors)

The paper discusses a number of recent studies investigating into the energy conservation and environmental impacts of electric heating systems (off-peak storage systems). In future, i.e. in and after 1993, off-peak storage systems will be evaluated through comparative analyses of oil-fired heating systems, pollutant emission analyses, the eventual success of pollution abatement measures designed for power plant emissions to keep within the legal limiting values, and the possible abandonment of pollution abatement measures abroad. Opposing the electric heating studies submitted by the electric power industry the author refutes their favourable results referring to the palliative selection of inadequate or vague parameters. Assuming average power plant pollutant emissions of 180 mg/standard/sup 3/ the nitrogen oxide emissions of oil-fired heating systems (134.1 kg/Tj) are found to be below those of off-peak storage systems (242.5 kg/Tj).

A 21-year-old woman developed midesophageal stricture two weeks after ingestion of aluminium phosphide (AlP) tablets. Aluminium phosphide is a lethal protoplasmic toxin and is also the most common cause of suicidal poisoning in northern India. Upper gastrointestinal endoscopy (UGIE) showed a tight esophageal stricture 29?cm from the incisors with a circumferential ulcer. Dilatation up to 17?mm was done using Savary-Gilliard dilators. She had repeated dilatations three times at nearly two-week intervals. Endovascular treatment of intrahepatic inferior vena cava obstruction from malignant hepatocellular tumor thrombus utilizing Luminexx self-expanding nitinol stents

We wanted to determine the technical and clinical efficacy of placing a self-expandable PTFE-covered nitinol stent for the management of inoperable malignant biliary obstruction. Thirty six patients with inoperable malignant biliary obstructions were treated by placement of self-expandable PTFE-covered nitinol stents (S and G Biotech Corporation, Seongnam, Korea). Clinical evaluation was done with assessment of the serum bilirubin and alkaline phosphatase levels, which were measured before and after stent placement within 1 week, at 1 month and at 3 months. The patient survival rate and stent patency rate were calculated with performing Kaplan-Meier survival analysis. Successful stent placement was achieved in all the patients without procedure-related complication. Pancreatitis as an early complication occurred in two cases. The health politics and policy communities are still struggling with the question of "what went wrong" in the 1993-94 health care reform effort. Here I identify which Americans were politically active and inactive during the health care reform debate to explore the role political participation may have had in determining the outcome of the debate. Using data from a national and California random-sample telephone surveys, and controlling for other demographic attributes, I found that those who engaged in political activity specifically related to health care reform were disproportionately more likely to be self-identified conservatives, less likely to favor an employer mandate plan, more likely to be fifty to sixty-four years old, more likely to be men, and more likely to have greater interest in and knowledge of the health care issue. Gastric dilatation-volvulus (GDV) is a life-threatening condition in dogs and other species in which the stomach dilates and rotates on itself. The etiology of the disease is multi-factorial, but explicit precipitating causes are unknown. This study sought to determine if there was a significant association between changes in hourly-measured temperature and/or atmospheric pressure and the occurrence of GDV in the population of high-risk working dogs in Texas. The odds of a day being a GDV day, given certain temperature and atmospheric pressure conditions for that day or the day before, was estimated using logistic regression models. There were 57 days in which GDV(s) occurred, representing 2.60% of the days in the 6-year study period. Six different groups of non-smoking young male subjects were studied separately for 18 consecutive days each in a closed controlled-environmental human exposure chamber. Each group was subjected to a 5-day control period in fresh air followed successively by an 8-day period of continuous exposure to 50 ppm, 15 ppm or 0 ppm (control) by volume of carbon monoxide (CO) in air, and a 5-day recovery period in fresh air. The subjects performed a 1-h auditory vigilance task every day at the same time of day in a fixed qualitative, quantitative, and temporal relationship with food intake, consumption of stimulating beverages, physical activity, and sleep. It was concluded that such CO exposure, involving the continuous carriage of carboxyhaemoglobin loads up to 7%, was without significant effect on auditory vigilance.

The mortality in undrained abdominal abscesses is high with a mortality rate ranging between 45 and 100%. The outcome in abdominal abscesses, however, has improved due to advances in image guided percutaneous interventional techniques. The main indications for the catheter drainage include treatment or palliation of sepsis associated with an infected fluid collection, and alleviation of the symptoms that may be caused by fluid collections by virtue of their size, like pancreatic pseudocele or lymphocele. The single liver abscesses may be drained with ultrasound guidance only, whereas the multiple abscesses usually require computed tomography (CT) guidance and placement of multiple catheters. Rapid corrosion of PWR steam generator carbon steel support structures and consequential denting of steam generator tubes led to considerable investigation of the mechanism of corrosion and for palliatives. The basic mechanism is now fairly well understood. Chloride present in the boiler feedwater from condenser leakage is concentrated in the support plate crevice as a result of the superheat present. This leads to the formation of a low pH acid ferrous chloride environment either through the hydrolysis of a weak base chloride such as MgCl_2 or the combined action of a neutral chloride with an oxidizing agent. Estradiol enhances neurogenesis in the dentate gyri of adult male meadow voles by increasing the survival of young granule neurons.

Pulmonary fibrosis was produced in mice treated with butylated hydroxytoluene (BHT) 400 mg/kg and immediately exposed to 80% oxygen for 3 days. This treatment regimen resulted in 47% mortality. Surviving mice exhibited significant accumulations of pulmonary collagen as evidence by increases in total lung hydroxyproline levels. The administration of indomethacin (4 mg/kg/day) on days 1-6 after BHT decreased mortality to 14% and diminished the accumulation of collagen in lung tissue. Indomethacin also enhanced survival when administered on days 1-3 after BHT/O/sub 2/ but had no effect on lung collagen levels. Treatment with indomethacin on days 4-6 after BHT had no beneficial effect. Effects of epidermal growth factor on bone formation and resorption in vivo

In this 150-day study, chicken manure, kenaf, and white rot fungus were added to soil microcosms in an attempt to enhance the degradation of polychlorinated biphenyls. The soil was contaminated with commercial PCB mixtures. Dishes were ammended with 5% dry weight chicken manure, 1% dry weight kenaf, and 1% dry weight kenaf plus Phanerochaete chrysosporium inoculant. PCB concentrations were determined at 30 day intervals by soxhlet extraction and gas chromatography analyses. Preliminary results of microbial populations and PCB degradation are presented. The research agenda in ICU telemedicine: a statement from the Critical Care Societies Collaborative.

For the performance assessment of radioactive and hazardous waste disposal sites, risk assessments are usually performed for the long term, i.e., over an interval in space and time for which one can predict movement and behavior of toxic agents in the environment. This approach is based on at least three implicit assumptions: One, that the engineering layout will take care of the immediate endangerment of potential receptors; two, that one has carefully evaluated just how far out in space and time the models can be extrapolated, and three, that one can evaluate potential health effects for very low exposures. A few of these aspects will be discussed here in the framework of the scientific method.

ICU telemedicine uses audiovisual conferencing technology to provide critical care from a remote location. Research is needed to best define the optimal use of ICU telemedicine, but efforts are hindered by methodological challenges and the lack of an organized delivery approach. We convened an interdisciplinary working group to develop a research agenda in ICU telemedicine, addressing both methodological and knowledge gaps in the field. To best inform clinical decision-making and health policy, future research should be organized around a conceptual framework that enables consistent descriptions of both the study setting and the telemedicine intervention. PURPOSE We wanted to examine how coordinated care is implemented in primary care practices to address patients' health behavior change needs. METHODS Site visit notes, documents, interviews, and online implementation diaries were collected from July 2005 to September 2007 from practice-based research networks (PBRNs) participating in Prescription for Health: Promoting Healthy Behaviors in Primary Care Research Networks (P4H). An iterative group process was used to conduct a cross-case comparative analysis of 9 interventions. Published patient outcomes reports from P4H interventions were referenced to provide information on intervention effectiveness. We report the results of a worldwide campaign to observe WZ Sagittae during its 2001 superoutburst. After a 23-year slumber at V=15.5, the star rose within 2 days to a peak brightness of 8.2, and showed a main eruption lasting 25 days. The return to quiescence was punctuated by 12 small eruptions, of ~1 mag amplitude and 2 day recurrence time; these "echo outbursts" are of uncertain origin, but somewhat resemble the normal outbursts of dwarf novae. After 52 days, the star began a slow decline to quiescence. Copper-65-absorption by men fed intrinsically and extrinsically labeled whole wheat bread

The half-life of "1"0"9Cd was measured by following the decay of sources from a radiochemically pure solution with two different measuring systems: an ionisation chamber and a high-purity germanium (HPGe) detector. The measurements were performed over a period of 3.6 years, i.e. about 2.8 half-lives of "1"0"9Cd. The resulting half-life values and detailed uncertainty budgets (k=1) are presented for both systems. The result obtained with the ionisation chamber, 462.36 (33) days, and the one obtained with the HPGe detector, 461.92 (76) days, are mutually consistent. Six men were fed a diet composed of conventional foods with all bread as whole wheat bread. Effects of "2"3"9Pu administered at 9 days of gestation on hematologic development of the rat

The in vivo toxic effects of hexavalent chromium (20 mg/liter) on hemolymph glucose, tissue glycogen, total free sugars, and active and total phosphorylases of an edible, freshwater crab Barytelphusa guerini were studied. In a 15-day exposure span followed by a 15-day postexposure recovery, the time-course alterations in these constituent segments of the glycogen metabolism indicate an inconsistent depletion in metabolite levels and elevated enzyme activities during exposure period as well as hyperglycemia. An insignificant recovery was observed in these parameters on the 15th day of the postexposure phase.

Injection of pregnant rats with monomeric "2"3"9Pu after 9 days of gestation decreased their leukocyte and reticulocyte counts at 5 and 10 days postexposure. Most of the fetal hematologic enumerative values were unaffected by injection of monomeric "2"3"9Pu. There was, however, a major change in the maturation of the cells of the erythroid series, as indicated by a difference in the distribution between cell types. Biosynthesis of hemoglobin and some of its electrophoretic fractions in red cells of peripheral blood and spleen of irradiated (650 R) rats has been studied. Hemoglobin synthesis is found to be most drastically inhibited in the first and second fractions on the first and eighth days after irradiation and in the fifth and sixth fractions on the eighth day (less expressed). The synthesis is restored on the twelfth day, the process under study proceeding more slowly in the above-mentioned fractions than in others. In the course of radiation sickness, the biosynthesis of certain hemoglobin fractions varies differently in the hemoglobin-synthesizing cells of peripheral blood than in the cells of spleenic erythroid series.

Effect of local irradiation on longitudinal bone growth in the rat. A tetracycline labelling investigation

The biological half-life of "1"3"7Cs in the total body of human subjects was determined in 23 individuals of Japanese male adult in their normal works by measuring amount of "1"3"7Cs in both their total body and daily urine in the same period. For the group, the value was determined by averaging the half-lives for individuals, by comparing the mean body burden and the mean daily urinary excretion, or by applying a curve fitting method to the body burden estimate. The biological half-life averaged 86 days, ranging from 50 to 161 days. The averages of the biological half-lives for the group were 83, 87 and 82 days in the different periods of observation. By the curve fitting method, 85 days was found for the group. The biological half-life for the individuals depended on both body weight and age, to a lesser extent, of the subjects. (author).

Early Loading after 21 Days of Healing of Nonsubmerged Titanium Implants with a Chemically Modified Sandblasted and Acid-Etched Surface: Two-Year Results of a Prospective Two-Center Study

Young rats were given a single irradiation dose (0.5, 2, 5 or 8 Gy) to the left knee-joint. The right unexposed knee-joint served as control. The animals were decapitated 1.5, 3, 7, 14 or 30 days after the irradiation. Longitudinal bone growth in the tibial epiphysis was established using tetracycline as an intravital marker. During the first 1.5 days after irradiation with 5 and 8 Gy, the growth was slightly inhibited (6-7%). Maximum growth retardation (20%) was found 7 to 14 days after irradiation with 5 and 8 Gy. No consistent effect occurred after 0.5 and 2 Gy. Between 14 to 30 days following irradiation growth was normalized.

In this study, apoptosis was identified by TUNEL staining and then related to cell proliferation, as determined by Ki-67 staining. The apoptotic index was at its highest at 8 days of involution, whereas the proliferation index was at its highest during lactation. One of the isolates was able to decolorize Terasil black dye under aerobic conditions in the presence of an exogenous carbon source after 5 days. Glucose or starch (%1 ea) are essential for decolorization but the process proceeds faster in the presence of 0.5% yeast extract. The prevalence, management and outcome for acute wounds identified in a wound care survey within one English health care district

Total skin electron beam therapy (TSEB) was used in the treatment of 33 patients with lymphoma and 13 patients with leukemia involving extensive segments of the skin surface. Twenty-two of 23 had lesions as a primary manifestation of lymphoma (primary cutaneous lymphoma-PCL) and 11 developed cutaneous lesions following disseminated nodal lymphoma (secondary cutaneous lymphoma-SCL). A once weekly fractionation scheme was employed to irradiate the entire skin surface with 3.5 to 4 MeV electron beam from a 6 MeV linear accelerator. During each weekly session, 400 rad were delivered to the entire skin and a complete course consisted of 4-6 consecutive weekly sessions. The Effects of Capitation on Outpatient Mental Health Episodes of Children

Thai Family Health Routines: scale development and psychometric testing.

We analyzed the effects of the Colorado Medicaid Capitation Program on the duration and services of over 21,000 outpatient mental health episodes for young children. The study spanned a three year period before and after capitation was implemented, and compared episodes of outpatient care for children from 14 capitated Community Mental Health Centers (CMHCs) with those from three CMHCs that remained fee-for-service. Proportional hazards regressions revealed that capitation resulted in an increase in outpatient episode duration, over and above similar effects for the comparison fee-for-service episodes. The hypothesis that there were longer capitated episodes for children who were hospitalized was not supported. Children with behavioral and anxiety disorders had longer capitated episodes. The aims of the study were to develop and test the psychometric properties of the Thai Family Health Routines (TFHR) scale, a 70-item self-report questionnaire used to measure the health of Thai families through their routine behaviors in daily life. Development of the TFHR was based on the structural domains of Denham's Family Health Model. The TFHR scale was initially composed of 85 items and tested on 1,040 families living in the central region of Thailand. The confirmatory factor analysis, with an acceptable factor structure model, yielded 70 items aligned with six factors: self-care, safety and prevention, mental health behavior, family care, family caregiving, and illness care routines. Indications were that risk for mental health disorders, including Post-Traumatic Stress Disorder (PTSD), may be elevated in residents, and that this risk may continue for some time. Patients may be de-prioritising their mental health issues when these become normalised throughout the city's population. The effective half-life based on urine bioassay results of Wolsung NPP's worker was calculated. The effective half-life for tritiated water vapour obtained was 5 {approx} 9 days. Upsetting categories? The consequences of pharmacogenomics for making knowledge-based reimbursement decisions in Sweden

The urine test strip is the most common test used to detect ketones in veterinary patients, but it can underestimate the degree of ketonuria and hence, ketonemia. Additionally, adequate urine samples for analysis may be difficult to obtain from dehydrated animals. The standard method used to detect and monitor ketonemia in human medicine is measurement of serum or whole blood beta-hydroxybutyrate (?HOB). A point-of-care (POC) analyzer has been validated for this purpose in humans. This study compared the accuracy of the POC device to an enzymatic reaction laboratory method for measurement of ?HOB in dogs. Although the POC sensor tended to overestimate ?HOB concentrations, there was good correlation (R(2) = 0.96) and good agreement between the 2 methods with a bias +/- precision of 0.0860 +/- 0.3410 mmol/L ?HOB. We present evidence for the existence of two ~35 day clocks in the Her X-1/HZ Her binary system. ~35 day modulations are observed 1) in the Turn-On cycles with two on- and two off-states, and 2) in the changing shape of the pulse profiles which re-appears regularly. The two ways of counting the 35 day cycles are generally in synchronization. To maintain operational readiness, military personnel engage in vigorous physical and training activities that carry risk for injury. A 1-year prospective cohort study, starting April 1996, was conducted at Fort Bragg, North Carolina among 1,965 members of the 82nd Airborne Division to quantify musculoskeletal injuries. Information collected included type of injury, site, circumstances, and resultant limited duty days. These soldiers suffered 508 overuse injuries (including 38 stress fractures), 1,415 traumatic injuries (including 100 fractures), and 101 unclassified injuries. For 30 minutes, two-day-old suckling mice, Jcl:ICR strain, were put into a chamber which was constantly flushed with 100% CO_2 gas. After the exposure to asphyxia, 29% of the mice survived. New concept to optimize emergency diagnoses in patients with multiple injuries

A filter for extracting solids and liquids from gaseous combustion exhausts comprises a textile filter element formed of polyacrylonitrile fibers oxidized by careful heat treatment and comprising approximately 61% carbon after treatment. The filter may be woven, knitted or formed as a batting of randomly-oriented fibers. Abrasion resistant coatings and lubricants for the fibers are disclosed. The filter material is especially useful as a filter bag.

Monoclonal antibody imaging in malignant and benign gastrointestinal diseases

To optimize the emergency care of patients with multiple injuries, a special emergency room with integrated radiological equipment has been developed. It consists of a linear array of a computed tomography system, a mobile C-arm with an integrated scattered radiation grid for conventional films and a C-arm fluorographic system. Incidents of major damage to steam turbines

The utilization of health care services in children and adolescents with chronic benign pain was studied in a Dutch population sample of 254 chronic pain sufferers aged 0-18 years. Children and adolescents who had reported chronic pain (continuous or recurrent pain >3 months) in our previous prevalence study were asked to keep a 3-week diary on their pain and to fill out questionnaires on background factors, health care use and the impact of pain. Parent ratings were used for children aged 0-11 years, self-report was used in adolescents (12-18 years). In a 3-month period, in 53.4% of the cases medication was used for pain, and general practitioners and specialists were consulted for pain in 31.1% and 13.9% of subjects, respectively. Influence of local waste burning on atmospheric aerosol properties in urban environment

A laser spectrometer for the ethylenediaminetetra-acetic acid (EDTA) titration of magnesium or calcium ions that is designed around a handheld laser pointer as the source and a photoresistor as the detector is developed. Aerosols affect the radiative energy budget on both the regional and global scales. The wavelength-dependent aerosol optical depth (AOD) is a fundamental determinant of the amount by which extra-terrestrial incoming sunlight and outgoing terrestrial radiation are being attenuated in the atmosphere. The present study addresses the influence of local waste burning on aerosol characteristics, black carbon (BC) aerosol mass concentration and spectral solar irradiance using ground-based measurements over the tropical urban environment of Hyderabad, India. Antiperoxidative and antiinflammatory effect of Sida cordifolia Linn. on quinolinic acid induced neurotoxicity.

The effect of prenatal cimetidine exposure (PreCM) on the duration of pentobarbital-induced narcosis (DPN) was assessed in immature (14- and 28-day old) and adult (50-60-day old) male and female rats. PreCM exposure was accomplished by treating mothers with cimetidine (CM) (20 mg/kg, ip) daily for the last two days of gestation and then (0.01% in drinking water) throughout lactation. Pregnant mothers of untreated offspring (Con) received saline. PreCM decreased DPN to 505 +/- 33 min (from 611 +/- 23 min in Con) and 393 +/- 190 min (from 686 +/- 44 min in Con) in 14-day old male and female rats, respectively. The elevation of blood levels of zinc protoporphyrin in mice following whole body irradiation

Forty-one patients were treated with epidural neurolysis using 50 % ethyl alcohol 2 ml. Thirty eight patients were suffering from cancer pain and three patients were complaining of chronic benign pain. Alcohol block was repeated 2.3 times (mean) in the same patient. Thirty patients were followed after the treatment. Forty-seven percent of the patients reported 70 % or greater pain relief and 20 % of the patients reported about 50 % pain relief. Duration of pain relief was from 9 days to 203 days with a mean duration of 54 days. Adverse effects were reported 43 % of the patients. There is no miserable adverse effect. Radon concentration measurements at water reservoirs

Elevation of zinc protoporphyrin (ZPP) levels in the blood has served as an indicator of lead poisoning and iron deficiency anemia for many years. We have discovered that sublethal doses of whole body irradiation with x-rays also elevates ZPP 2-3-fold over normal levels. The ZPP level does not begin to increase until days 12-14 postirradiation and peaks between days 18 and 20 before returning to normal levels between days 28 and 35. Increasing the radiation dose delays the onset of the rise in ZPP, but does not affect the magnitude of the elevation. At lethal doses, ZPP elevation is not observed. Neither of the two previously described mechanisms that cause elevations of ZPP, namely iron deficiency and inhibition of ferrochelatase, are responsible for the radiation-induced elevation of ZPP. The elevation of ZPP appears to be correlated with the recovery of the hematopoietic system from radiation injury.

Ground water is treated in the Czech Republic so that small water tanks are built above each water source to serve the primary ground water treatment; water so pretreated is then concentrated in large basins for subsequent treatment. Some water tanks where the first contact of the ground water with air takes place were selected as sites predisposed to radon accumulation. The examination was carried out near the town of Jihlava, where the bedrock contains slightly elevated radium concentrations. The average radon concentrations lay within the region of 2-4 kBq/m"3; the instantaneous values, however, exhibited appreciable periodical variations during the day. The relatively high radon concentrations will not pose a marked hazard for the personnel because the employees only reside at the sites in question for 10 to 15 minutes within 2 days, not for the whole working day

Prevention of death from ingested ruthenium-106 by colectomy

The effects of repeated low-level nerve agent exposure on animal performance and lethality are a major concern of USAF Medical Research. This concern has generated interest in the role that pretreatment drugs such as pyridostigmine may play during simultaneous exposure to soman. This role was investigated by recording lethality, weights, symptoms, and cholinesterase (ChE) inhibition in rats chronically exposed to 4 levels of soman (32, 39, 48 or 59 micrograms/kg/day) while simultaneously receiving 10.0 milligrams/kg/day pyridostigmine, 1.0 milligrams/kg/day pyridostigmine, or vehicle via an osmotic pump. No effect (either protective or detractive) was found in the soman-poisoned animals due to the presence or absence of pyridostigmine. However, rat blood biochemistry is different from that of the primate, warranting further study in the primate before extrapolation to man.

Pineal and photoperiodic influences on fat deposition, pelage, and testicular activity in male meadow voles.

Two dogs were given 3.0 mCi/kg body weight of "1"0"6Ru-"1"0"6Rh and two were given 4.0 mCi/kg. At 5 days after treatment, one dog from each dose level was colectomized. The dog given the larger dose but not surgically treated was killed in extremis due to acute lower bowel injury at 18 days. The other dog, given 3.0 mCi/kg but not surgically treated, was killed in a moribund condition at 145 days after dosing. The rectum had perforated. The colectomized dogs recovered from intestinal injury and showed no radiation damage at the termination of the study.

Oral intake of purple passion fruit peel extract reduces pain and stiffness and improves physical function in adult patients with knee osteoarthritis

Pinealectomy completely prevented gonadal regression as well as reduction in body weight and white adipose tissue content of the gonadal and retroperitoneal fat deposits in male meadow voles transferred from long to short day lengths. Pineal influences on pelage characteristics depended on which parameter was assessed. For instance, the increase in guard hair length observed in short-day control voles was blocked by pinealectomy; however, a similar increase in underhair length was unaffected by removal of the pineal gland. Photoperiod-dependent changes in fat deposition, testicular activity, and guard hair length presumably rely on altered pineal secretory activity to transduce the effects of day length on the neuroendocrine axis; however, mechanisms independent of pineal activity may be capable of mediating photoperiodic control of underhair growth. The hypothesis tested in this study was that the passion fruit peel extract (PFP), a flavonoid-rich dietary supplement, would reduce symptoms due to knee OA. Thirty-three OA patients were enrolled in a randomized, double-blind, placebo-controlled trial with parallel-group design. Effect of some chemicals on the radiation-induced lipid peroxidation

The possible long-term effects of chronic radiation exposure have been studied. Rats were given tritiated drinking water (3 #mu#Ci/ml) before and during pregnancy and thereafter continuously through five generations. The brains were studied at 30 and 120 days postnatally. Significant decreases in weight and DNA and protein contents of specific parts of the brain were found, the most pronounced decreases being in the diencephalon. The decrease in protein content were more pronounced than the decreases in DNA content; the decreases were also more pronounced at 30 days than at 120 days, and more pronounced in the fith generation than in previous generations.

Effect of age-at-weaning on digestive capacity of white seabream (Diplodus sargus)

The effect of exposure to gamma rays on LPO contents in liver, spleen and kidney of mice and the anti-lipid-peroxidation of preparations of chinonin, tannic acid and squalene were reported. All these preparations were given by intragastric perfusion. For LPO, a modified method of TBA spectrophotometry was used for determination. The results show that LPO contents in tissues increased 72 h after whole-body exposure of mice to gamma rays, that in range of 0-4 Gy, there was a linear relationship between radiation doses and LPO contents, and that all three preparations were effective in reduction of the LPO content in liver, spleen and kidney of mice, with chinonin (at dose of 0.35 mg/mouse#centre dot#day) being more effective than tannic acid (at dose of 15 mg/mouse#centre dot#day) and squalene (at dose of 0.6 mg/mouse#centre dot#day).

Biochemical alterations induced by Zn and Cd individually or in combination in the serum of Oreochromis niloticus

The possible long term effects of chronic radiation exposure have been studied. Rats were given tritiated drinking water (3#mu#Ci/ml) before pregnancy, during pregnancy and threafter, continuously through 5 generations. The brains were studied at 30 and 120 days postnatally. Significant decreases in weight, DNA and protein contents of specific parts of the brain were found, the most pronounced decreases being in the diencephalon. The decreases in protein content were more pronounced than the decreases in DNA content; the decreases were also more pronounced at 30 days than at 120 days, and more pronounced in the fifth generation than in previous generations. An attempt was made to explain these complex phenomena on the basis of known periods of proliferation of various brain cells, damage to mothers themselves in each generation, and damage and repair of nucleic acids caused by radioactivity.

Anatomical Description of the Female Reproductive Organ and Radiation Induced Histological changes of Ovary of Melon fly, Bactrocera cucurbitae (Coq.) (Diptera: Tephritidae)

The results of radon concentration measurements in the near surface air at San Nicolas Island, California, are presented for five different measurement sessions between May 1978 and July 1980. Emphasis is on the previously unreported results for January and July 1980. For six of eight days during the January session the measured radon concentration was greater than 40 picocuries per cm, corresponding to a continental air mass. Only the first day involved a maritime air mass with radon concentration less than about 6 picocuries per cm. In contrast to all the previous sessions, maritime air dominated the ten day session in July 1980, with radon concentrations generally less than about 5 picocuries per cm.

Production and evaluation of labeled bisphosphonates with 153Sm, 166Ho and 177Lu as therapeutic radiopharmaceuticals

Application of gamma radiation as a physical method of disinfestations against melon flies was recognized as a potential quarantine treatment. At 50 Gy, oocytes showed degeneration one day after treatment whereas seven-day-old oocytes did not differ greatly in appearance from control groups. Abnormal enlargement of trophocyte cells and vacuolization of oocytes occurred predominantly following the treatment with 100 and 150 Gy. One day after treatment with 150 Gy trophocytes underwent hypertrophy and hyperplasia. Irradiation at 100 and 150 Gy reduced the fertility to almost zero percent in the female melon flies.(authors)

[Legionella pneumonia caused by aspiration of hot spring water after sarin exposure].

Purpose: To determine the value of MRI in the postoperativ evaluation of a singular ventricle compared to echocardiography and cardiac catheterization. Materials and methods: Thirty-one patients (range: 6 months to 30 years) with a functional single ventricle following palliative corrective operations. Five patients had a Blalock-Taussig-Shunt, 8 patients a Glenn-Anastomosis and 18 a cavopulmonary shunt (6 with classic Fontan-Circulation, 12 with modified cavopulmonary anastomosis). The results in terms of postoperative morphologic changes were compared to percutaneous echocardiography (31/31) and cardiac catheterization (6/31). A 72-year-old man was exposed to the sarin gas attack in a Tokyo subway on March 20 th, 1995. After exposure, he noticed eye discomfort, chest tightness, headache and weakness of the lower limbs and oropharyngeal muscles. Despite these symptoms, he visited a hot spring on the same day with his family. On March 25 th, his muscle weakness progressed, and a low grade fever appeared. The fading of irradiated blue-colored pearls

The diagnosis of idiopathic pulmonary hemosiderosis (IPH) may be elusive. A 6-year-old boy had microcytic hypochromic anemia and a hemolytic component. Hemosiderin-laden macrophages were not found in the gastric aspirate. He had no pulmonary signs or symptoms. Extensive hematologic and roentgenologic investigations failed to reveal the cause of the anemia. Quantitative serial scintigraphic scanning showed significant (35%) pulmonary sequestration of autologous erythrocytes labeled with sodium chromate Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy spece Cr51. This paper analyses the complex, non-linear effects of spot price drivers in wholesale electricity markets: their intra-day dynamics and transient irregularities. The context is the UK market, after the reforms introduced in March 2001, analysed with an original set of price drivers reflecting economic, technical, strategic, risk, behavioural and market design effects. Models are estimated separately as daily time-series of the 48 half-hourly trading periods. All coefficients exhibit substantial intra-day variation, relating to the heterogeneity of operating plants and market design aspects. 1. Gamma globulin metabolism and distribution were studied employing rabbit gamma globulin (RGG)I(131) 24 times in 13 control rabbits. Similar studies were performed before and during the ananmestic response in 4 rabbits previously sensitized with a polyvalent pneumococcal vaccine. 2. During the ananmestic response, gamma-globulin levels increased from 1.0 to 6.0 gm/100 ml, and the gamma-globulin pool increased from 0.7 to 4.7 gm/kg. There was no change in the intravascular-extravascular partition of gamma globulin. 3. Gamma globulin degradation increased from 0.06 to 0.33 gm/kg/day during the 28 days of the immunization period while gamma globulin synthesis increased even further to average 0.47 gm/kg/day. Exercise training associated with robust conditioning can be useful for the study of molecular mechanisms underlying exercise-induced cardiac hypertrophy. A swimming apparatus is described to control training regimens in terms of duration, load, and frequency of exercise. Mice were submitted to 60- vs 90-min session/day, once vs twice a day, with 2 or 4% of the weight of the mouse or no workload attached to the tail, for 4 vs 6 weeks of exercise training. Blood pressure was unchanged in all groups while resting heart rate decreased in the trained groups (8-18%). Changes in contents of the two kinds of neuropeptides have been measured in the brains of postnatal G57BL/6J strain mice that received accumulative doses of 0, 0.05, 0.10 or 0.30 Gy respectively in utero from tritium #beta#-irradiation during the mother mice's pregnancy from the 12.5th day of gestation to the day of delivery. Dose response relationship for alteration in somatostatin content due to exposure to 0.10 Gy or above was observed, and it was also noted that 0.30 Gy could cause decrease in arginine vasopressin in hypothalamus. These results were consistent with findings in research on brain development and reflex behavior in the literature. 0.10 Gy exposure may represent a threshold for exposure to tritiated water within the range from day 12.5 of gestation to the day of delivery using change in content of somatostatin in hypophysis as a parameter.

Optimization and automation

Water-soluble, ethanol-insoluble extracts prepared from several medicinal herbs of the Araliaceae and other families were tested for their radioprotective activity by either measuring 30 days survival rates (a commonly applied method) or counting the thrombocytes 14 days after irradiation (an improved method) in mice. The extracts were i.p. injected once within 4 min after the whole body X-ray irradiation. Of 12 materials examined, Aralia elata, Angelica acutiloba and Morus bombysis seemed to be good starting materials for preparing radioprotective substance(s) because of their relatively high specific activity and high yields of the extracts. Ancient cosmological tachyons in the present-day world

The geodesic equation for space-like objects moving along a circular trajectory in the expanding universe is considered. Our analysis leads to the conclusion that ancient cosmological tachyons may exist in the present-day world and may play an important role in (i) the internal structure of hadrons conceived as nonlocal objects called strings, (ii) the T-symmetry violation observed in the weak K-decays, (iii) the multidimensional unified field theories of Kaluza-Klein type, and in (iv) the classical models of charged particles which combine ordinary electromagnetism with a self-interacting version of Newtonian gravity. 18 refs.
