Amphiphysin is a protein concentrated in neuronal synapses and peripherally associated with neurotransmitter vesicles. It is expressed in many neurons of the central and peripheral nervous systems, in the adrenal medulla, in the anterior and posterior pituitary, in cell lines of the endocrine pancreas, and in spermatocytes. Its subcellular localization and tissue distribution indicate a potential involvement in mechanisms of regulated exocytosis. In this work, we address the problem of road interpretation for driver assistance based on an early cognitive vision system. The structure of a road and the relevant traf?c are interpreted in terms of ego-motion estimation of the car, independently moving objects on the road, lane markers and large scale maps of the road. We make use of temporal and spatial disambiguation mechanisms to increase the reliability of visually extracted 2D and 3D information. This information is then used to interpret the layout of the road by using lane markers that are detected via Bayesian reasoning. We also estimate the ego-motion of the car which is used to create large scale maps of the road and also to detect independently moving objects. Sample results for the presented algorithms are shown on a stereo image sequence, that has been collected from a structured road.

Pain is necessary for survival but chronic pain is disabling and causes significant health and economic problems. This study provides an understanding of the future for spinal cord stimulation. Stimulation by means of chronically implanted electrodes, was carried out in 200 patients with pain of varied benign organic etiology. In 177 of them, pain was confined to the failed back syndrome. Most patients were referred by a Pain Management Service. 226 epidural implants were used: 80 unipolar, 59 Resume, 12 bipolar, and 75 quadripolar. Patients were followed for periods of 6 months to 12 years, with a mean follow-up of 44 months. 84 patients (42%) were able to control their pain by stimulation alone, 22 patients (11%) needed occasional analgesic supplements along with their stimulation program. Sex differences in pain are frequently reported in the literature. However, less is known about possible sex differences in the experience of pain secondary to a disability. The current study explored these issues in persons with limb loss (n = 335, 72% men) who were recruited as part of a postal survey. Participants provided ratings of phantom limb pain (PLP), residual limb pain (RLP), and general pain intensity. Participants also completed measures of pain-related interference, catastrophizing, coping, and beliefs. Results indicated that a greater proportion of males than females (86% vs 77%, respectively) reported the presence of PLP; however, this difference was no longer prominent when cause of limb loss was controlled. In response to an earlier published paper by Fordyce, some assumptions underlying the behavior management paradigm of chronic pain are critically discussed. While operant treatment has proved successful, the conclusion that operant factors play an important role in the development and maintenance of chronic benign pain is debated. Some empirical studies, regularly used to demonstrate this role, are re-evaluated. An alternative theory is proposed for chronic pain behavior, in which the role of a lower tolerance to proprioceptive stimuli, which may include more than just pain stimuli, is emphasized. The method consits of drilling exploratory wells and measurement of the physical parameters of the stratum liquid. The piezometric levels are periodically determined simultaneously in all the observation wells during a change in the atmospheric pressure, and the presence of a remoteness of the gas deposit from the well is judged from the size of the increments in the piezometric levels intime. The determinations of the piezometric levels in the wells are conducted during an abrupt change in atmospheric pressure, for instance, during the replacement of a cyclone by an anticyclone or the reverse. Noninvasive and simplified methods for estimating regional cerebral blood flow (CBF) and regional partition coefficient (#lambda#) using the inhalation of stable xenon (Xe"s) and computed tomographic (CT) scanning are described. Thirty percent Xe"s in 70% oxygen was inhaled for 240 seconds and exhaled for 160 seconds during serial CT scanning without denitrogenation in 26 patients with cerebrovascular diseases and four volunteer controls. During the investigation, the end-tidal Xe"s concentration was continuously monitored with a thermoconductivity analyzer to determine the build-up range (A value) and build-up rate constant (K value) of the artery by the curve fitting method. Forty-one patients were treated with epidural neurolysis using 50 % ethyl alcohol 2 ml. Thirty eight patients were suffering from cancer pain and three patients were complaining of chronic benign pain. Alcohol block was repeated 2.3 times (mean) in the same patient. Thirty patients were followed after the treatment. Forty-seven percent of the patients reported 70 % or greater pain relief and 20 % of the patients reported about 50 % pain relief. Duration of pain relief was from 9 days to 203 days with a mean duration of 54 days. Adverse effects were reported 43 % of the patients. There is no miserable adverse effect. Adverse effect reported most was pain with epidural injection of drugs after the alcohol block had been performed. PMID:8544293

In the last 40 years, significant advances have been made in the understanding of the neurophysiologic processes involved in the experience of trauma and pain. This knowledge, together with the rapid growth and understanding in the behavioral health sciences, has expanded to include a much better appreciation of how these fields are converging and contribute to a process called neuroplasticity. These basic mechanisms common to all patients have important implications for clinical outcome and for improving clinical practice. The utilization of health care services in children and adolescents with chronic benign pain was studied in a Dutch population sample of 254 chronic pain sufferers aged 0-18 years. Children and adolescents who had reported chronic pain (continuous or recurrent pain >3 months) in our previous prevalence study were asked to keep a 3-week diary on their pain and to fill out questionnaires on background factors, health care use and the impact of pain. Parent ratings were used for children aged 0-11 years, self-report was used in adolescents (12-18 years). Determining the type of matter that is inside a neutron star (NS) has been a long-standing goal of astrophysics. Despite this, most of the NS equations of state (EOS) that predict maximum masses in the range 1.4-2.8 solar masses are still viable. Most of the precise NS mass measurements that have been made to date show values close to 1.4 solar masses, but a reliable measurement of an over-massive NS would constrain the EOS possibilities. Here, we investigate how optical astrometry at the microarcsecond level can be used to map out the orbits of High-Mass X-ray Binaries (HMXBs), leading to tight constraints on NS masses. Back pain from osteoporosis is commonly related to compression fractures. The patient with vertebral compression fractures additionally suffers from an ongoing risk of recurrent fractures and postural abnormalities that can result in impaired respiratory function, leading to increased risk of morbidity. Weakened back muscles, especially the back extensor group, are felt to contribute significantly to this risk. The combination of pharmacologic and nonpharmacologic interventions, through physical activity, exercise, and modalities, are potential interventions that could be used to help reduce the pain of osteoporotic compression fractures as well as potentially reducing recurrent fracture rates.

A new method of administration of an opioid was recently registered: fentanyl transdermal (brand name: Durogesic), intended particularly for the indication range 'pain in cancer'. Fentanyl is lipid-soluble so that deposition in the skin takes place and the biological half-life is approximately 20 hours after removal of the plaster. It is safe to start on a basis of an equianalgesic conversion of 100:1 in relation to oral morphine, although this may entail some risk of fentanyl under dosage. The dose adjustment time is 12-24 hours before a constant fentanyl level is reached; therefore, after attaching the first sticking plaster, the original morphine dose should be continued for another 12 hours. In addition, the patient may, if necessary, be given supplementary morphine preferably as a short-acting drug. There seems to be no clear indication for transdermal fentanyl either in neuropathic pain or in chronic benign pain. ...

High correlations have been reported between RLP and the stump neuromas following amputation. Many treatment methods including physical therapy, medications, and interventions, have been used with limited success. Pulsed radiofrequency ablation (PRFA) has shown promise in treating neuropathic pain because of the inhibition of evoked synaptic activity. We present 4 amputees who were treated with PRFA after failing conservative management for their RLP and PLP. It is a particularly popular approach in genomic and proteomic sequence analysis. Numerous mappings of symbolic sequences have been proposed for various applications. It is unclear however whether the processing of symbolic data provides an artifact of the numerical mapping or is an inherent property of the symbolic data. This issue has been long ignored in the engineering and scientific literature. Morphine sulfate (MS Contin), a proven analgesic in the treatment of cancer pain and chronic benign pain, seems to be a good analgesic for the treatment of burn pain. MS Contin is morphine sulfate incorporated in a wax cellulose matrix delivery system. This wax cellulose delivery system gives MS Contin its duration of action. Ten patients were enrolled in an open-labeled, nonrandomized study. The study was designed to examine the analgesic efficacy of MS Contin in the burn population. Each patient remained in the study for 6 days. The efficacy of the analgesic regimen was subjectively measured by the visual pain scale. Studies of the effectiveness of operant, relaxation, cognitive, and multimodal behavioural approaches to the treatment of chronic benign pain other than headache were evaluated. In general, the quality of the studies was poor, and most investigations lacked appropriate and adequate control conditions, outcome measures, and/or follow-ups. While outcome reports for all four behavioural treatments have been mainly positive, few data were found which conclusively demonstrate that any of the approaches are effective or that they are the treatment of choice. The data do, however, imply that behavioural approaches may help patients lead more normal and productive lives. To investigate possible microdamage/microwear to dental enamel caused by such usage, puncture-crushing experiments were carried out on single human teeth, using an Instron compression apparatus on the following six test materials: Makapansgat Limeworks chert (e.g., taphonomy), fresh steer longbone, mongongo nuts, Grewia berries, Carob beans, and wild-onion bulbs. Pairs of extracted unworn third molars were utilized, with one tooth acting as the control. The teeth were mounted, ultrasonically cleaned, and two-stage replicas made with a vinyl polysiloxane elastomer and araldite epoxy resin. A noninvasive, simplified method using inhalation of stable xenon (Xe"s) and computed tomographic (CT) scanning to estimate regional cerebral blood flow (rCBF) and regional partition coefficient (r#lambda#) is described. Twenty-four patients with cerebrovascular occlusive disease and six volunteer controls inhaled 30% Xe"s and 70% oxygen for 180 seconds and exhaled for 144 seconds during serial CT scanning without denitrogenation. The end-tidal Xe"s concentration was continuously monitored with a thermoconductivity analyzer to determine the build-up range (A value) and build-up rate constant (K value) for arteries with the curve fitting method. The aim of this review is to evaluate the role of inflammatory spine disease in patients with chronic back pain. The contribution of imaging modalities for the diagnostic evaluation of back pain is discussed. A systematic literature search based on the classification of seronegative spondyloarthropathies and rheumatoid arthritis was performed. The results of this search and the experiences in a large collective of rheumatological patients are analyzed. The prevalence of rheumatoid arthritis (1-2%) is comparable to that of spondyloarthropathies (1.9%). The etiology of these entities is not fully elucidated. Magnetic resonance imaging is increasingly used for early detection and surveillance of therapy with TNF-#alpha# antagonists. Snapping or grating scapula is a condition of the shoulder girdle in which the patient complains of pain and an audible snapping sound which may be associated with pain. We evaluated the diagnostic value of the computed tomography (CT) scans using the clinical diagnosis of a grating scapula as the gold standard. This retrospective study reports the sensitivity, specificity, likelihood ratio and receiver operator characteristic curve for the CT scans and the k value as a measure of the interobserver agreement. None of the parameters validated the examination. We therefore concluded that CT scans are inappropriate for differentiation between a grating scapula and a normally functioning shoulder except in cases where bony abnormalities are present. (orig.)

A method is proposed for evaluating reliability of planning and calculation of the reliability factor with current planning of open pit mining operations. For a quantitative evaluation of the reliability of fulfilling the planned assignment, laws were defined for distribution of the quality characteristics of the coal. After establishing that coal ash content follows the normal distribution law, the probability was defined that the actual output of coal is greater or equal to the planned. Calculation of the factor of reliability of the current planning of open mining operations makes it possible to determine the optimum planning for this period and the probability of fulfillment of the planned assignments. The technique of planning with regard for reliability makes it possible to select the most reliable variant of the plan within the planning period.

The overall objective of this research project is to develop a technical basis for flexible piping designs which will improve piping reliability and minimize the use of pipe supports, snubbers, and pipe whip restraints. The current study was conducted to establish the necessary groundwork based on the piping reliability analysis. A confirmatory piping reliability assessment indicated that removing rigid supports and snubbers tends to either improve or affect very little the piping reliability. We then investigated a couple of changes to be implemented in Regulatory Guide (RG) 1.61 and RG 1.122 aimed at more flexible piping design. We concluded that these changes substantially reduce calculated piping responses and allows piping redesigns with significant reduction in number of supports and snubbers without violating ASME code requirements. Cell-penetrating peptides (CPPs) are useful tools for the delivery of hydrophilic bioactive molecules, such as peptides, proteins, and oligonucleotides, across the cell membrane. To realize the delivery of therapeutic macromolecules by CPPs, the CPPs are required to show resistance to protease and no cytotoxicity. In order to produce potent non-toxic and protease-resistant CPPs with high cellular uptake, we designed an amphipathic helix peptide using ?-aminoisobutyric acid (Aib, U) and named it MAP(Aib). In the MAP(Aib) molecule, five Aib residues are aligned on the hydrophobic face of the helix and five lysine (K) residues are aligned on the hydrophilic face. MAP(Aib) showed potent resistance to trypsin and pronase compared with MAP, an amphipathic helix peptide formed by usual amino acids. Probing of high density cosmid grids will enable integration of the ESTs into cosmid contigs and location of the cosmid contigs on the YAC contig. A hn-cDNA library has been constructed from the hybrid CY18 which contains chromosome 16 as the only human chromosome. Government facilities such as the US Department of Energy's Savannah River Plant (SRP) near Aiken, South Carolina, often use remote sensing data to assist in environmental management. Airborne multispectral scanner (MSS) data have been acquired at SRP since 1981. Various types of remote sensing data have been used to map and characterize wetlands. Regional Landsat MSS and TM satellite data have been used for wetlands mapping by various government agencies and private organizations. Furthermore, SPOT MSS data are becoming available and provide opportunities for increased spacial resolution and temporal coverage for wetlands mapping. This paper summarizes the initial results from using five dates of SPOT MSS data from April through October, 1987, as a means to monitor seasonal wetland changes in freshwater wetlands of the SRP. 11 refs., 4 figs.

In previous studies we have demonstrated that prion protein (PrP) interacts with tubulin and disrupts microtubular cytoskeleton by inducing tubulin oligomerization. These observations may explain the molecular mechanism of toxicity of cytoplasmic PrP in transmissible spongiform encephalopathies (TSEs). Here, we check whether microtubule associated proteins (MAPs) that regulate microtubule stability, influence the PrP-induced oligomerization of tubulin. We show that tubulin preparations depleted of MAPs are more prone to oligomerization by PrP than those containing traces of MAPs. Tau protein, a major neuronal member of the MAPs family, reduces the effect of PrP. Importantly, phosphorylation of Tau abolishes its ability to affect the PrP-induced oligomerization of tubulin. A two-phase flow regime map is developed experimentally and theoretically for a vertical hexagonal flow channel with and without a 36-finned rod hexagonal bundle. This type of flow channel is of interest to MAPLE-type nuclear research reactors. The flow regime maps are determined by visual observations and observation of waveforms shown by a capacitance-type void fraction meter. The experimental results show that the inclusion of the finned hexagonal bundle shifts the flow regime transition boundaries toward higher water flow rates. Existing flow regime maps based on pipe flow require slight modifications when applied to the hexagonal flow channel with and without a MAPLE-type finned hexagonal bundle. The proposed theoretical model agrees well with experimental results.

Pyogenic sacroiliitis is a rare entity in children. Diagnosis is often delayed because of its variable clinical presentation, low suspicion by the examining physician, and rare findings on radiographs. Delayed diagnosis, however, results in complications such as iliopsoas abscess. We report the case of a 12-year-old girl hospitalized with a 21-day history of fever, pain in the left iliac fossa, and flexion contracture of the hip. On examination, she had fever (38.9 ^oC), psoitis, localized tenderness at the left sacroiliac joint, and pain elicited by lateral compression of the pelvis. The abdominal examination was normal. The theory of reliability is used to develop statistical field data on malfunctions of units of sucker rod well pumps (UShSN). The indices of reliability of the UShSN are applied for establishment of the cause-effect links between the operating factors. Dependences of operating time on the depth of suspension of the pumps, mode of pumping out, degree of flooding of the oil, and twisting of the shafts are established for conditions of specific fields. The obtained relationships can be used in selecting and optimizing the work of the UShSN.

During their life, intervertebral discs undergo degenerative changes which need not necessarily become lesions. Conventional X-rays do not provide information on the condition of the intervertebral discs, so MRT or CT are recommended as primary techniques as they are non-invasive. Invasive methods like myelography and especially discography should not be used unless a case is highly problematic. This research project is to develop a technical basis for flexible piping designs which will improve piping reliability and minimize the use of pipe supports, snubbers, and pipe whip restraints. This study indicated that piping design can be made more reliable by some reduction of rigid supports and/or snubbers. This study also confirmed that the malfunction of pipe whip restraints introduced higher thermal stresses and tended to reduce the overall piping reliability. Finally, our results indicated that supports in a flexible piping design may need to be reevaluated and that the elimination of pipe supports which are close to components should be done with care in order to minimize the impact on the component reliability.

The reliability analysis of the Fast Flux Test Facility reactor shutdown system was reevaluated. Failure information based on five years of plant operating experience was used to verify original reliability numbers or to establish new ones. Also, system modifications made subsequent to performance of the original analysis were incorporated into the reevaluation. Reliability calculations and sensitivity analyses were performed using a commercially available spreadsheet on a personal computer. The spreadsheet was configured so that future failures could be tracked and compared with expected failures. A number of recommendations resulted from the reevaluation including both increased and decreased surveillance intervals. All recommendations were based on meeting or exceeding existing reliability goals. Considerable cost savings will be incurred upon implementation of the recommendations.

The author applies the GO methodology to reliability analysis of the offsite power supply system of Daya Bay NPP. The direct quantitative calculation formulas of the stable reliability target of the system with shared signals and the dynamic calculation formulas of the state probability for the unit with two states are derived. The method to solve the fault event sets of the system is also presented and all the fault event sets of the outer power supply system and their failure probability are obtained. The resumption reliability of the offsite power supply system after the stability failure of the power net is also calculated. The result shows that the GO methodology is very simple and useful in the stable and dynamic reliability analysis of the repairable system

A new leaf-sequencing approach has been developed that is designed to reduce the number of required beam segments for step-and-shoot intensity modulated radiation therapy (IMRT). This approach to leaf sequencing is called continuous-intensity-map-optimization (CIMO). Using a simulated annealing algorithm, CIMO seeks to minimize differences between the optimized and sequenced intensity maps. Two distinguishing features of the CIMO algorithm are (1) CIMO does not require that each optimized intensity map be clustered into discrete levels and (2) CIMO is not rule-based but rather simultaneously optimizes both the aperture shapes and weights. The mapping of the magnetic flux bundle from the center cell to the Plasma Potential Control plates (PPC) on the end fan of the Tandem Mirror Experiment Upgrade (TMX-U), was improved by the addition of trim coils (12,000 amp-turns) on each side of each end fan next to the pump beam magnetic shields. The coils' axes are oriented perpendicular to the machine centerline. These coils made the necessary corrections to the field-line mapping, while keeping the field in the nearby pump beam magnetic shield below the saturation threshold. This paper briefly describes the problem, discusses the design as it evolved, and presents the results of the field testing. A brief description of the improvement in the machine performance is also included.

An optimal deployment pattern of flux mapping detectors for a Canada uranium-deuterium (CANDU)-600 pressurized heavy water reactor (PHWR) is determined by obtaining an optimal feedback relationship between flux measurements and zone controllers. The reactor core is modeled with a time-dependent two-group, two-dimensional diffusion equation, and flux perturbation are expressed by model expansions. The modal expansion coefficients are used as elements of the state vector representing the system dynamics. An optimal feedback matrix connecting the flux measurement vector to the control vector is derived by minimizing a quadratic performance index involving both the state and control vectors. We obtain the detector effectiveness in terms of the optimal feedback matrix and determine optimal detector locations for the Wolsung Unit 1 reactor in Korea. Knowledge of subsurface temperatures obtained from heat flow and equilibrium temperature measurements were used as a basis for evaluation of the usefulness of the non-equilibrium temperatures of oil and gas wells for geothermal exploration. We find that the data in the Geothermal Gradient Map of North America do not conform to the thermal regime of Nebraska; and, that if the BHT data were the only data used, we would have underestimated our resource by about 80%. Differences in geological sections and heat flow between Nebraska and the U.S. Gulf coast where the correction data for the USGS map were obtained are the reasons for the discrepency. Some significant inaccuracies in the BHT data may not be correctable.

The effect of weak dissipation on drift orbits has been investigated making use of a simple mapping model in a helical magnetic field. It is found that, after many mapping iterations, any orbit tends to an attractor forming a vortex line even with very small dissipation. The convergence is faster for larger dissipation, i.e., the number of iteration N to converge within a certain distance from the attractor is inversely proportional to the amount of the dissipation. Although the behavior of orbits completely change, the basic stability characteristics of the system does not change, i.e, the coordinate of the attractors are determined by the stable fixed points in the area preserving system because the dissipation is very small. Since wide range of orbits are concentrated around the attractors after many toroidal circulations, a pinch effect is created by a small dissipation. A new prototype model system named AirGIS has been developed to support local authorities in air quality management for big Danish cities. The system is based on the Danish operational street pollution model (OSPM), technical and cadastral digital maps and Danish national administrative databases on buildings, cadastres and populations. It applies a geographic information system (GIS). AirGIS estimates ambient air pollution levels at high temporal and spatial resolutions. The model system enables mapping of traffic emissions, air quality levels and human exposures at residence addresses, at workplace addresses and in streets. Mapping and scenario results can be compared with air quality limits. Impact assessment of traffic air pollution abatement measures can also be carried out. (author)

Corrosion is increasingly becoming an important factor reducing the reliability of many nuclear power plant components. The significance is evaluated of corrosion phenomena with respect to the reliability of primary circuit components of LWR's, viz., the reactor pressure vessel, primary piping, steam generator, and fuel elements. The mechanism of corrosion phenomena is explained and methods of minimizing their effects are presented. An analysis is made of the needs to solve the corrosion problems of nuclear power plants from the point of view of Czechoslovak producers and research and development activities. International cooperation is reviewed and main problems are formulated on which the solution of corrosion problems of structural materials used in WWER type nuclear power plants should be focussed. In the universe there are two different kinds of uncertain phenomena: stochastic and fuzzy or vague. Both uncertain phenomena have been found in structural problems. Therefore, rational decisions concerning the design of marine structures cannot be made without resorting to the methods which can take the uncertainties into account. The fundamental concept of structural fuzzy reliability problems and the methods to give the fuzzy solution are introduced in the paper. Based on the principle of fuzzy decision-making the method to obtain the crisp solution of structural fuzzy reliability analysis is proposed by means of the Bound Search method. Illustrative numerical examples, ship bottom plates under slamming impact pressures are solved. The influence of variation of allowable tolerances in the fuzzy reliability indexes is discussed.

This paper presents a probabilistic approach for structural robustness assessment for a timber structure built a few years ago. The robustness analysis is based on a structural reliability based framework for robustness and a simplified mechanical system modelling of a timber truss system. A complex timber structure with a large number of failure modes is modelled with only a few dominant failure modes. First, a component based robustness analysis is performed based on the reliability indices of the remaining elements after the removal of selected critical elements. The robustness is expressed and evaluated by a robustness index. Next, the robustness is assessed using system reliability indices where the probabilistic failure model is modelled by a series system of parallel systems.

The paper gives an introduction to reliability assessment of reliquefaction systems for boil-off gas (BOG) on LNG carriers with focus on redundancy optimization and maintenance strategies. The reliability modeling is based on a time-dependent Markov approach. Four different system options are studied, with varying degree of redundancy. Failures in the reliquefaction system may require flaring of the BOG, and the associated cost is compared with the cost of introducing redundancy and the cost of onboard maintenance. A model for maintenance optimization is developed and exemplified on a main unit of the reliquefaction system. Reliability and maintenance cost data for reliquefaction systems on LNG ships are very scarce. The input data have been collected from the best available data sources and adjusted by expert judgement. A tailor-made computer program has been developed and may be supplied to interested readers.
