Understanding Center for Medicare and Medicaid Services (CMS) documentation and coding rules is challenging for most physicians. To accurately bill for clinical services, physicians must learn a system that may initially seem daunting, but is in fact governed by a small number of straightforward rules. The Evaluation and Management (E/M) guidelines for all service codes specify 3 components: history, examination, and medical decision-making, each with a defined set of elements or characteristics. Service coding is based on the level of care supported by the number of history and examination elements and the complexity of decision-making. In Niigata University Hospital, most of conventional radiography were changed to Fuji computed Radiography (FCR) from July 1988. To evaluate its clinical utility, questionnaires relating to FCR image were distributed to all physicians including non-radiologists in the hospital after three months. The ratio of the physicians evaluated FCR image to be better than conventional image was 75% in chest radiography, 64% in abdominal radiography. FCR tomography of bone, joint, cranium and chest was also evaluated to be better than conventional by most of the physicians. Reduced size of the film and difficulty in detecting interstitial shadows of the lung were mentioned to be the fault of FCR. (author).

The United States deployed 118 military working dogs (MWDs) to the Persian Gulf theater during Operations Desert Shield and Desert Storm. This study is a retrospective descriptive study of medical records of these deployed dogs, with the objective to determine whether there were infectious or parasitic diseases with a zoonotic potential in a sentinel population of MWDs that may be of concern to Persian Gulf veterans. Fifty-one percent of visits to veterinary treatment facilities during deployment were for illness or injury. Potential zoonotic conditions accounted for 21% of the total visits, 41% of the "sick-call" visits, and 63% of presentations for illness to veterinary treatment facilities. This study did not determine whether the diseases treated were transmitted between MWDs and the troops. The traveler participated in the discussions about the physics that can be learned in the experiments being planned, those presently running, or those recently completed by a growing number of major laboratories in Europe and the USA at the 'Workshop on NN Bremsstrahlung and Polarization Observables', where many representatives of these experimental programs were present. The traveler's research in this field is supported by a DOE Grant, and she has pioneered the early theoretical NN(lambda) treatments. In addition, she visited the Vrije University to collaborate with Professor Henk Blok on proton scattering from Zr-90 and neighboring nuclei. The traveler also visited NIKEF and discussed possible future experiments of mutual interest to be done at that laboratory.

Study Design An economic decision tree is used to predict outcomes and costs to compare these two procedures from a US Medicaid perspective over a 5-year time horizon. The aim of the study was to assess the usefulness of artificial neural networks (ANN) application in evaluation of scintimammography in the context of clinical data in the diagnosis of breast cancer. The results produced by ANN were compared with the diagnosis of two independent observers, nuclear medicine specialists. Material and methods: The clinical data and the numerical values derived from scintimammograms of 103 patients were the material for the study. The reference method was the result of histopathology study (core biopsy and /or FNB). Results: The overall sensitivity of physician diagnosis was 78% with specificity of 72%. The ANN produced 71% sensitivity and specificity of 73%. The physicians and ANN results were not significantly different (p=0.4619). Conclusions: Artificial neutral networks are useful tool in clinical diagnosis of breast cancer. (authors)

A survey of wetlands on the Oak Ridge Reservation (ORR) was conducted in 1990. Wetlands occurring on ORR were identified using National Wetlands Inventory (NWI) maps and field surveys. More than 120 sites were visited and 90 wetlands were identified. Wetland types on ORR included emergent communities in shallow embayments on reservoirs, emergent and aquatic communities in ponds, forested wetland on low ground along major creeks, and wet meadows and marshes associated with streams and seeps. Vascular plant species occurring on sites visited were inventoried, and 57 species were added to the checklist of vascular plants on ORR. Three species listed as rare in Tennessee were discovered on ORR during the wetlands survey. The survey provided an intensive ground truth of the wetlands identified by NWI and offered an indication of wetlands that the NWI remote sensing techniques did not detect.

The objective of this project, which was sponsored by EPRI, is to record the practical knowledge on the O M of a number of the domestic and foreign pumped storage plants which has been accumulated by their operating staff. Civil, geotechnical, mechanical and electrical aspects of O M of the thirty-five plants visited are discussed, and some general conclusions, as warranted by the commonality of the problems, are drawn. These conclusions may be useful to the planners and designers of future pumped storage projects. The tapeworm Spirometra mansonoides infects man worldwide, particularly in Asian countries. Rarely, the central nervous system is involved; such a case is presented here. In the total of 12 reported cases, including the case described, the worm presented clinically as a mass suspicious for neoplasm or chronic abscess cavity. Surgical removal was invariably curative in each case. Although infrequent, the possibility of tapeworm infection should be entertained in the evaluation of intracranial masses in patients who have visited exotic locales. PMID:2191089

The provincial ministry of health has taken over a dialysis center in La Plata, Argentina, following the discovery that at least 20 of the clinic's 34 regular patients have been infected with the human immunodeficiency virus (HIV). Catheters of the clinic's dialysis machines were found to be contaminated with HIV. The 5 physicians associated with the Centro Modelo de Dialisis have been discharged for negligence. 20 of the regular patients also contracted hepatitis C. Argentinian law stipulates that filters for the same patient must be changed after 4 times and prohibits the use of the same vessel to administer medication to more than 1 patient. Testimony from nurses at the La Plata clinic revealed that filters were reused for several patients and basic sanitary procedures were not followed. In 1990, 33 kidney disease patients at a dialysis center in Cordoba became infected with HIV and it was discovered that filters had been used up to 8 times for different ...

Periureteral or subtrigonal injection of dextranomer/hyaluronic acid (Dx/HA) copolymer (Deflux, Q-Med, Uppsala, Sweden) is an increasingly common endoscopic treatment for vesicoureteral reflux. We report a confusing radiographic finding of bilateral calcified Dx/HA injections initially thought to represent bilateral distal ureteral stones in a boy who presented with intermittent periumbilical pain. Urologists, radiologists, and emergency room physicians should be aware of the potential for calcification of ureteral implants of Dx/HA, and of the potentially confusing radiographic images that may result. In recent years, several groups, including the physician assistant, health information management, ophthalmic medical technician/technologist, and athletic training review committees, have left the Commission on Accreditation of Allied Health Education Programs (CAAHEP) system to form their own, profession-specific accrediting bodies. Their motivation was typically a desire for greater professional visibility and autonomy. Combined, these professions represented one third of the CAAHEP's programs. This article reviews the history of allied health and examines current attempts to bring cohesion and identity to this increasingly fractured segment of the U.S. health care system. PMID:18444439

There have been many questions about the costs of providing medical services. These questions have resulted in the development of Resource-Based Relative Values Scale (RBRVS) at the national level. Policymakers view RBRVS as a potential tool to pay physicians. The questions, about the cost of operations of HSE-2 at Los Alamos National Laboratory, revolve around doing routine occupation health physicals for the Laboratory. Generally, these questions are concerned about why the costs appear to be high relative to what private physician suggests the costs should be. Health physicists from the Brookhaven National Laboratory (BNL) visited the Savannah River Site (SRS) as one of 12 facilities operated by the Department of Energy (DOE) contractors with annual collective dose equivalents greater than 100 person-rem (100 person-cSv). Their charter was to review, evaluate and summarize as low as reasonably achievable (ALARA) techniques, methods and practices as implemented. This presentation gives an overview of the two selected ALARA practices implemented at the SRS: Administrative Exposure Limits and Goal Setting. These dose control methods are used to assure that individual and collective occupational doses are ALARA and within regulatory limits.

CASE REPORT: A 10 year-old Caucasian girl was referred to the Dental Department, at Glasgow Royal Hospital for Sick Children by her Paediatric Nephrologist Consultant. The patient was being teased by her peers over her markedly discoloured teeth. The dental history revealed that the patient was a regular dental attendee from an early age. Rapid and reliable transmission of nuclear medicine studies using conventional telephone lines and commercially available modems and computer systems has been accomplished through use of software developed within the authors' hospital. Original digital images of all-night and weekend studies, acquired on any of the acquisition computers from different manufacturers, are now routinely sent for remote reading at the physician's home. Data, software, and letters are routinely exchanged using modems and standard telephone lines with a sister institution in Haifa, Israel. The software has been designed to achieve no loss data compression and minimal turnaround time loss. Thus, an average lung perfusion image or gallbladder study requires about 1-3 minutes of transmission time. Full analysis and display software is available on the remote computer.

The goal of the trip was to obtain information on German developments, experience, and possibilities in the field of biomass gasification. The participants were representatives of Dutch parties in the energy sector: waste sector, manufacturers, producers, policy makers and consultants. The most important feature was the visit to plants that were in operation. Due in particular to the new EEG (Emeuerbare-Energien-Gesetz/Renewable Energy) legislation, German policy makers have created an initial market for sustainable energy with a degree of success. We reconsider stochastic convergence analyses of particle swarm optimisation, and point out that previously obtained parameter conditions are not always sufficient to guarantee mean square convergence to a local optimum. We show that stagnation can in fact occur for non-trivial configurations in non-optimal parts of the search space, even for simple functions like SPHERE. The convergence properties of the basic PSO may in these situations be detrimental to the goal of optimisation, to discover a sufficiently good solution within reasonable time. To characterise optimisation ability of algorithms, we suggest the expected first hitting time (FHT), i.e., the time until a search point in the vicinity of the optimum is visited. It is shown that a basic PSO may have infinite expected FHT, while an algorithm introduced here, the Noisy PSO, has finite expected FHT on some functions.

Controversy now rages as the Australian government is claiming pound33 million for this purpose from the United Kingdom to compensate the Maralinga Tjarutja for the loss of access to their traditional lands. They claim that the UK clean-up operation was inadequate and that much plutonium was spread over the land, rather than contained in pits. As compared with terrestrial circuits, the communication satellite possesses superior characteristics such as wide area coverage, broadcasting, high capacity, and robustness to disasters. Utilizing the narrow band channel (64 kbps) of the geostationary satellite JCSAT 1 located at the altitude of 36,000 km above the equator, the authors investigated satellite-relayed medical imagings by video signals, with bone scintigraphy as a model. Each bone scintigraphy was taken by a handy-video camera, digitalized and transmitted from faculty of technology located at 25 kilometers apart from our department. Clear bone scintigraphy was obtained via satellite, as seen on the view box. Eight nuclear physicians evaluated 20 cases of bone scintigraphy. A 72-year-old man was exposed to the sarin gas attack in a Tokyo subway on March 20 th, 1995. After exposure, he noticed eye discomfort, chest tightness, headache and weakness of the lower limbs and oropharyngeal muscles. Despite these symptoms, he visited a hot spring on the same day with his family. On March 25 th, his muscle weakness progressed, and a low grade fever appeared. We present measurements of the transmission of radio/microwave pulses through salt in the Cote Blanche salt mine operated by the North American Salt Company in St. Mary Parish, Louisiana. These results are from data taken in the southwestern region of the 1500 ft. (457 m) deep level of the mine on our third and most recent visit to the mine. We transmitted and received a fast, high-power, broadband pulse from within three vertical boreholes that were drilled to depths of 100 ft. (30 m) and 200 ft. below the 1500 ft. level using three different pairs of dipole antennas whose bandwidths span 125 to 900 MHz. By measuring the relative strength of the received pulses between boreholes with separations of 50 m and 169 m, we deduce the attenuation of the signal attributed to the salt medium. We fit the frequency dependence of the attenuation to a power law and find the best fit field attenuation lengths to be 93 \\pm 7 m at 150 MHz, 63 \\pm 3 m at 300 MHz, and 36 \\pm 2 m ...

A one-year technology-transfer project involving ERDA installations and Hawaii consisted of sending teams from the Lawrence Livermore Laboratory on week-long field trips every two months to test the effectiveness of different methods of transferring technology information from federal sources to civilian clients. The team was questioned primarily on non-energy matters, and the energy questions asked related mostly to individuals or small industries. The team responed to all questions and found that a wide range of knowledge was more effective than having a sequence of experts. Hawaiians considered current major ERDA projects to be irrelevant to their needs. The team was most successful on a one-to-one basis because large groups and state agencies tend to be more policy- than action-oriented. Personal followup was considered essential. The purpose of this study was to determine whether low doses of carbon monoxide (CO) exacerbate myocardial ischemia during a progressive exercise test. The effect of CO exposure was evaluated using the objective measure of time to development of electrocardiographic changes indicative of ischemia and the subjective measure of time to onset of angina. Sixty-three male subjects (41-75 years) with well-documented coronary artery disease, who had exertional angina pectoris and ischemic ST-segment changes in their electrocardiograms, were studied. Results from three randomized, double-blind test visits (room air, low and high CO) were compared. It is customary in the United States (US) to add isopropyl alcohol to the bulk shipment of water-wet high explosives RDX and HMX. The explosives are packed in cloth bags which are placed in plastic-lined fiber drums. The addition of alcohol presumably prevents mildewing of cloth bags and freezing of the wet explosives in cold weather. In Europe, however, these explosives are shipped in polyethylene-lined fiber drums with not less than 15% water only, even in cold weather. Water-wet frozen explosives have not proved to be any more sensitive than its unfrozen counterpart and no mildew problem has been encountered. Thirty-two asthmatic volunteers 8 to 16 yr of age, recruited through local schools and private physicians, were exposed in a chamber to clean air (control condition) and to sulfuric acid aerosol at a low concentration (46 +/- 11 micrograms/m3; mean +/- SD) and at a high concentration (127 +/- 21 micrograms/m3). Acid aerosols had mass median aerodynamic diameters near 0.5 microns with geometric standard deviations near 1.9. Temperature was 21 degrees C, and relative humidity was near 50%. Subjects were exposed with unencumbered oronasal breathing for 30 min at rest plus 10 min at moderate exercise (ventilation rate approximately 20 L/min/m2 of body surface). Some of the cylinders are more than 40 years old, and approximately 17,500 are considered problem cylinders because their physical integrity is questionable. These cylinders require an annual visual inspection. The remainder of the 46,000-plus cylinders must be visually inspected every four years. Currently, the cylinder inspection program is extremely labor intensive. Because these inspections are accomplished visually, they may not be effective in the early detection of leaking cylinders. This report summarizes the results of a dioxin/furan emissions test of a secondary-copper-recovery cupola furnace equipped with an afterburner for hydrocarbon emissions control and two baghouses for particulate-emissions control. The cupola furnace is used for recovery of copper from telephone scrap and other copper-bearing materials. The test was No. 10 in a series of dioxin/furan emissions tests conducted under Tier 4 of the National Dioxin Study. The primary objective of Tier 4 is to determine if various combustion sources are sources of dioxin/or furan emissions. If any of the combustion sources are found to emit dioxin or furan, the secondary objective of Tier 4 is to quantify these emissions. The utilization of health care services in children and adolescents with chronic benign pain was studied in a Dutch population sample of 254 chronic pain sufferers aged 0-18 years. Children and adolescents who had reported chronic pain (continuous or recurrent pain >3 months) in our previous prevalence study were asked to keep a 3-week diary on their pain and to fill out questionnaires on background factors, health care use and the impact of pain. Parent ratings were used for children aged 0-11 years, self-report was used in adolescents (12-18 years). In a 3-month period, in 53.4% of the cases medication was used for pain, and general practitioners and specialists were consulted for pain in 31.1% and 13.9% of subjects, respectively. Physiotherapists, psychologists and alternative health providers were visited by 11.5, 2.8, and 4.0%, respectively. In the preceding year, 6.4% had been hospitalized due to pain. It utilizes the fuel designed for high temperature reactors operating in a relatively low temperature of PWR environment The 15 mm diameter spherical fuel elements are made of TRISO type microspheres embedded in graphite and cladded by SiC. The coolant flow transfers them from the fuel chamber into the core and become fixed forming a suspended core. Any accident signal will cut off the power to the coolant pump causing a stop in the flow. This study compared rCBF pattern in normal adult and normal children using statistical parametric mapping (SPM). The purpose of this study was to determine distribution pattern not seen visual analysis in both groups. Tc-99m ECD brain SPECT was performed in 12 normal adults (M:F=11:1, average age 35 year old) and 6 normal control children (M:F=4:2, 10.5{+-}3.1y) who visited psychiatry clinic to evaluate ADHD. Their brain SPECT revealed normal rCBF pattern in visual analysis and they were diagnosed clinically normal. Using SPM method, we compared normal adult group's SPECT images with those of 6 normal children subjects and measured the extent of the area with significant hypoperfusion and hyperperfusion (p<0.001, extent threshold=16). Current methods to calculate dose distributions with organ motion can be broadly classified as 'dose convolution' and 'fluence convolution' methods. In the former, a static dose distribution is convolved with the probability distribution function (PDF) that characterizes the motion. However, artifacts are produced near the surface and around inhomogeneities because the method assumes shift invariance. Fluence convolution avoids these artifacts by convolving the PDF with the incident fluence instead of the patient dose. In this paper we present an alternative method that improves the accuracy, generality as well as the speed of dose calculation with organ motion. The algorithm starts by sampling an isocenter point from a parametrically defined space curve corresponding to the patient-specific motion trajectory. A prospective, side-by-side comparison study of two different lasers for the treatment of solar- induced telangiectasia was carried out in 14 patients at the Beckman Laser Institute and Medical Clinic. The argon tunable dye laser (Coherent, Palo Alto, Calif.) was used in the method modified from Orenstein and Nelson to completely treat discrete telangiectasias on one cheek. Specifically, the argon tunable dye laser (ATDL) was set at 0.7 - 0.8 watts, 585 nm wavelength, shutter-pulsed at 0.1 second duration with a spot size of 0.1 mm, and individual vessels were 'traced out' with 4X loupe magnification. Each patient's opposite cheek was then treated in the standard fashion with the flashlamp pulsed dye laser (Candela, Natick, Mass.) using a technique similar to Polla's et al. Development and deployment of small-scale nuclear power reactors and their maintenance, monitoring, and control are part of the mission under the Small Modular Reactor (SMR) program. The objectives of this NERI-consortium research project are to investigate, develop, and validate advanced methods for sensing, controlling, monitoring, diagnosis, and prognosis of these reactors, and to demonstrate the methods with application to one of the proposed integral pressurized water reactors (IPWR).
