Since August 1984, the MITRE Corporation has been supporting the Naval Sea Systems Command (NAVSEA) and the Naval Medical Command (NAVMEDCOM) in their joint efforts to enhance the Navy Occupational Health Information Management System (NOHIMS). The goal of the enhancement effort was to create a comprehensive occupational health and safety system for Navy industrial facilities by expanding upon the original NOHIMS functions and adding modules for hazard deficiency abatement, hazardous-material control, injury claims and compensation, and safety and health training. To meet this goal, MITRE developed an enhanced industrial subsystem, referred to as the Occupational Safety and Health Record Keeping System (OSHRKS), using a prototyping approach and a public-domain data base-management software package, the Veterans Administration's (VA's) FileManager (FileMan).

The population doses and risks of stochastic effects from occupational exposures in Japan in 1978 were estimated on the basis of a nation-wide survey. The survey was conducted on annual collective dose equivalents by sex, age group and type of radiation work for about 82,500 workers other than the workers in nuclear power stations. The data on the workers in nuclear power stations were obtained from the offitial publication of the Japan Nuclear Safety Commission. The total number of workers except for nuclear power stations was estimated to be about 170,000 persons. Radiation works were subdivided as follows: medical works including dental; nondestructive inspectional; non-atomic energy industrial; research and educational; atomic energy industrial and nuclear power industrial. The OSHA's chemical occupational exposure standard for laboratories is an outgrowth of the previously issued Hazard Communication Standard. The standard relieves laboratories from complying with general industry standards but does not require compliance with specific guidelines. The heart of the standard is the creation of a Chemical Hygiene Plan (CHP) in every laboratory. The CHP addresses major issues such as safety equipment and procedures, work practices and training, the designation of a chemical hygiene officer, and the provision of medical consultation and examination for affected employees. This new standard, in effect as of 31 January, 1991, presents yet another regulatory challenge to laboratory managers but also will ensure a safer work environment for laboratory workers.

For the risk estimate of fatal malignancies, an effective dose was proposed on the basis of the assumption that the risk should be equal whether the whole body irradiated uniformly or whether there is non-uniform irradiation. The effective dose was defined by the product of organ or tissue doses and a weighting factor representing the proportion of risk factor for a fatal malignancy resulting from organ or tissue irradiation to the total malignant factor. The risk of malignancies can be derived by multiplying the malignant significant factor by the product of the risk factor and the effective dose. For the genetic risk, a significant factor was a relative child expectancy and organ or tissue doses were gonad doses. In consistency with the increasing requests from Member States for establishing radiotherapy programmes, an IAEA Technical Co-operation project was initiated in Latin America aimed at improving the physical aspects (as a complement to the clinical issues) of quality assurance in radiotherapy; this ARCAL XXX project (RLA/6/032) was classified as a Model Project of the IAEA. Among the important outcomes of the project were (i) the training of a considerable number of medical physicists in hospitals of the region, (ii) the development of a protocol for quality control procedures, and (iii) the organization of quality audit site visits (to the participant countries) where the implementation in hospitals of the developed quality control procedures is verified. The first part of the study contains a literature compilation of more than 50 original publications reporting the radiation induced effects in 17 different high polymer materials and glass which are relevant in the manufacturing of medical devices or packing materials. The results collected demonstrate that high energy radiation, i.e. gamma- or X-rays, causes various physical and chemical alterations in high polymer materials. A detailed summary and discussion of the results of the original publications is not included in the present report, it will be presented in the second part of the study. Collective dose equivalent and population dose from occupational exposures in Japan, 1988 were estimated on the basis of a nationwide survey. The survey was conducted on annual collective dose equivalents by sex, age group and type of radiation work for about 0.21 million workers except for the workers in nuclear power stations. The data on the workers in nuclear power stations were obtained from the official report of the Japan Nuclear Safety Commission. The U.S. Department of Energy (DOE) has funded major environmental technology developments over the past several years. One area that received significant attention is robotics, which has resulted in the development of a wide range of unique robotic systems tailored to the many tasks unique to the DOE complex. These systems are often used in highly hazardous environments, which reduces or eliminates worker exposures. The DOE, concurrent with the technology development initiative, also established and funded a 5-yr cooperative agreement intended to interface with the technology development community-with specific attention to the occupational safety and health aspects associated with individual technologies through human factors and hazard assessments. This program is now in its third year.

The focus of the Chernobyl Studies Project has now turned to the issue of health effects from the Chernobyl accident. Currently, we are involved in and making progress on the case-control and co-hort studies of thyroid diseases among Belarussian children. Dosimetric aspects are a fundamental part of these studies. We are working to implement similar studies in Ukraine. A major part of the effort of these projects is supporting these studies, both by providing methods and applications of dose reconstruction and by providing support and equipment for the medical teams.

Radiation therapy has had a significant and an expanded role in the management of cancer of the pancreas during the last decade. In particular, for locally advanced disease, radiation therapy has improved the median survival of patients to 1 year. Intraoperative electron beam therapy has been applied to unresectable and resectable pancreatic cancer in an attempt to enhance local control of disease and to improve patient survival. This paper presents a survey of the role of radiation therapy in treatment of cancer of the pancreas, provides information on the radiobiological aspects of this treatment modality and details the physical and dosimetric characteristics of intraoperative radiation therapy with electrons. Naturally occurring radionuclides are present in most material. The most common naturally occurring radionuclides in material are those of the uranium and thorium series and potassium-40. This material is commonly referred to as Naturally Occurring Radioactive Material (NORM). In some material the levels of naturally occurring radionuclides are significantly higher, to the extent that regulatory control may be required for radiation protection purposes. Regulation of NORM presents a range of new challenges for both regulators and operators. Unlike more traditional industries dealing with radionuclides, NORM industries have generally not had any radiological oversight and, for example, are not equipped for radiological monitoring. Some consumer goods containing NORM, which have not traditionally been considered as a radiological problem (such as some fertilizers), may require regulation and this may have social and economic consequences. The first chapter is a brief introduction to the technology of medical accelerators with emphasis on the electron beam production facilities. Chapter 2 describes the interaction processes at the atomic level once the electron beam enters the medium. Chapter 3 is concerned with the various properties of the electron beam purely from the clinical point of view. Government agencies responsible for protecting the public from the adverse effects of toxic chemicals must set priorities for research, regulatory action, protocol testing, and monitoring due to the vast number of toxic chemicals and the limited resources available to these agencies. The National Institute for Occupational Safety and Health (NIOSH) must set priorities for research on hazards encountered in the workplace. Priorities are also utilized by NIOSH in preparing criteria for recommended occupational standards which are forwarded to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, for possible promulgation. The OASIS Registry started annual collection of longitudinal data on patients on home parenteral nutrition (HPN) in 1984. This report describes outcome profiles on 1594 HPN patients in seven disease categories. Analysis showed clinical outcome was principally a reflection of the underlying diagnosis. Patients with Crohn's disease, ischemic bowel disease, motility disorders, radiation enteritis, and congenital bowel dysfunction all had a fairly long-term clinical outcome, whereas those with active cancer and acquired immunodeficiency syndrome (AIDS) had a short-term outcome. The long-term group had a 3-year survival rate of 65 to 80%, they averaged 2.6 complications requiring hospitalization per year, and 49% experienced complete rehabilitation. The short-term group had a mean survival of 6 months; they averaged 4.6 complications per year and about 15% experienced complete rehabilitation. Two hazard risk assessment matrices for the ranking of occupational health risks are described. The qualitative matrix uses qualitative measures of probability and consequence to determine risk assessment codes for hazard-disease combinations. A walk-through survey of an underground metalliferous mine and concentrator is used to demonstrate how the qualitative matrix can be applied to determine priorities for the control of occupational health hazards. The semi-quantitative matrix uses attributable risk as a quantitative measure of probability and uses qualitative measures of consequence. A practical application of this matrix is the determination of occupational health priorities using existing epidemiological studies. The increasing number of aspect mining techniques proposed in literature calls for a methodological way of comparing and combining them in order to assess, and improve on, their quality. This paper addresses this situation by proposing a common framework based on crosscutting concern sorts which allows for consistent assessment, comparison and combination of aspect mining techniques. The framework identifies a set of requirements that ensure homogeneity in formulating the mining goals, presenting the results and assessing their quality. We demonstrate feasibility of the approach by retrofitting an existing aspect mining technique to the framework, and by using it to design and implement two new mining techniques. The objective of this study was to describe trends in occupational fatalities among foreign-born workers. An analysis of fatal injuries among foreign-born workers in the US occurring from 1992 through 2007 was conducted using the Bureau of Labor Statistics? Census of Fatal Occupational Injuries. Recent research findings of epidemiologist Alice Stewart suggest that nuclear workers may be at risk of contracting cancer even though their measured occupational doses fall within current safety standards. Enhanced heat transfer in curved rectangular channels depends on at least two geometric factors, the radius of curvature of the duct and the aspect ratio, which is the width/depth ratio of the duct. In this work, an expression for optimum aspect ratio is derived mathematically from a correlation that includes both aspect ratio and bend curvature parameters. Enhancement factors are developed that show optimal, near-optimal, and nonoptimal aspect ratios for curved rectangular ducts. Predicted enhancements are within 25% of published literature results for individual designs, two of which show heat transfer enhancement of 200% over that of a straight duct with turbulent flow. The theoretical maximum enhancement is shown to be about 480% over a straight-duct, turbulent-flow situation. A reliable inventory control system was developed at the Fast Flux Test Facility (FFTF) to keep track of the occupancy of 900 refueling facility locations, to compile historical data on the movement of each reactor assembly, and to simulate assembly moves. The simulate capability is valuable because it allows verification of documents before they are issued for use in the plant, and eliminates the possibility of planning illegal or impossible moves. The system is installed on a UNIVAC 1100 computer and is maintained using a data base management system by Sperry Univac called MAPPER.

Theatres are the most complex of all auditorium structures environmentally. They usually have high heat loads, which are of a transient nature as audiences come and go, and from lighting which changes from scene to scene, and they generally have full or nearly full occupancy. Theatres also need to perform well acoustically, both for the spoken word and for music, and as sound amplification is less used than in other auditoria, background noise control is critically important. All these factors place constraints on the ventilation design, and if this is poor, it can lead to the deterioration of indoor air quality and thermal comfort. When evaluating the risks of oncogenesis and cancer mortality following exposure to the radiations of the atomic bombs (A-bombs), the medical X-ray doses received by the A-bomb survivors must also be estimated and considered. Using a human phantom, dosimetry was performed to estimate the X-ray doses received by A-bomb survivors during medical examinations at the Radiation Effects Foundation (RERF) as part of the long-term follow-up on the Adult Health Study (AHS). These examinations have been estimated to represent nearly 45% of the survivors' cumulative medical irradiation dose. Doses to the salivary glands, thyroid gland, lung, breast, stomach and colon were measured using thermoluminescent dosemeters. The results, which are reported here, will aid in estimating organ doses received by individual AHS participants. (Author).

One of the goals of the DARPA Virtual Soldier Project is to aid the field medic in the triage of a casualty. In Phase I, we are currently collecting 12 baseline experimental physiological variables and a cardiac gated Computed Tomography (CT) imagery for use in an prototyping a futuristic electronic medical record, the "Holomer". We are using physiological models and Kalman filtering to aid in diagnosis and predict outcomes in relation to cardiac injury. The physiological modeling introduces another few hundred variables. Reducing the complexity of the above into easy-to-read text to aid in the triage by the field medic is the challenge with multiple display solutions. A description of the possible techniques follows. This article is an update on the evidence supporting medications for adult ADHD, with particular emphasis on cardiovascular implications. Relevant clinical literature was sought using PubMed searches, with an emphasis on new reports from April 2009 to April 2011. This review describes the efficacy and general tolerability of stimulant and nonstimulant medications for adults with ADHD as seen in contemporary clinical trials.
