Multi-functional Bioactive Nano-structured Coatings for Load-Bearing Implants

Chinese herbs have been and still are widely used as important remedies in Oriental medicine. Over the recent years, a variety of biologically active constituents have been isolated from these sources and confirmed to have multifunctional activity in experimental studies. Honokiol is a small-molecule polyphenol isolated from the genus Magnolia. It is accompanied by other related polyphenols, including magnolol, with which it shares certain biological properties. Recently, honokiol and magnolol have been found to have anti-oxidative, anti-inflammatory, anti-tumor, and anti-microbial properties in preclinical models, without appreciable toxicity. These findings have increased interest in bringing honokiol and magnolol to the clinic as novel therapeutic agents in dermatology. The combination of functional and morphological imaging technologies such as positron emission tomography (PET) and X-ray computed tomography (CT) has shown its value in the clinical and preclinical field. However, CT provides only very limited soft-tissue contrast and exposes the examined patient or laboratory animal to a high X-ray radiation dose. In comparison to CT, magnetic resonance tomography (MRI) provides excellent soft-tissue contrast and allows for nuclear magnetic resonance spectroscopy (NMRS) or functional MRI (fMRI). Thus, the combination of PET and MRI has been pursued for several years. First approaches have succeeded using conventional photo multiplier tube (PMT) technology together with light fibers to transfer scintillation light away from the high magnetic field. Wood Plastic Composite (WPC) was prepared with simul (soft wood, density = 0.4g/cc) and butylmethacrylate (BMA) monomer using 10% methanol as the swelling agent. Effect of additives including (i) multifunctional acrylates such as tripropylene glycol diacrylate (TPGDA), trimethylol propane triacrylate (TMPTA) (ii) oligomer acrylates like the urethane (UA), epoxy (EA) and polyester (PEA) acrylates and (iii) N-vinyl pyrrolidone (NVP) was investigated using 1 to 3 Mrad dose at 0.8 Mrad/h. Synergistic increases in polymer loading yields was achieved in presence of the additives, particularly with the trifunctional acrylate (TMPTA). In addition, acid as well as urea were also used as co-additives and synergistic enhancement in yields of polymer loading were obtained. The radiation polymerization of multifunctional unsaturated polyester-monomer mixtures in wood forms interpenetrating network system. The mechanical resistance (compression, abrasion, hardness, etc.) of these composite materials are generally well over the original wood, however the impact strength is almost the same or even reduced, in comparison to the wood itself. An attempt is made using elastic polyester resins to produced wood-polyester composite materials with improved modulus of elasticity and impact properties. For the impregnation of European beech wood two types of elastic unsaturated polyester resins were used. The exothermic effect of radiation copolymerization of these resins in wood has been measured and the dose rate effects as well as hardening dose was determined. Felxural strength and impact properties were examined. Elastic unsaturated polyester resins improved the impact strength of wood composite materials. (author).

The model employs Monte-Carlo and Markov Chain algorithms that uses a weighted index to train and simulate the fuzzy hazard data sets which represents failure outcomes of risk component transient and non-transient systems. The report reviews the activity during the fourth year of Gas Research Institute's Gas Appliance Technology Center program. Work on gas appliances involved a conceptual table-top multi-function oven, development of an indirect-fired conveyor oven, and performance comparisons of smooth cooktops. Performance-evaluation methods for commercial ovens were evaluated. Design improvements were made to a direct-fired conveyor oven and a pulse-combustion deep-fat fryer. Performance of common vents were studied, and a device was designed to allow incorporation of appliances with positive vent pressures into such systems. Standards-related work included draft-hood capacities, flexible corrugated-connector corrosion, and sediment trap efficiencies. Heating-related work comprised conceptual design of a vented baseboard heater and a centralized heat-transfer fluid system.

Particularly lacking are details on how identified descending neurons integrate diverse sensory inputs to generate specific locomotor patterns. We have examined the operations of the principal neurons in an intriguing midbrain nucleus, the nucleus of the medial longitudinal fasciculus (nMLF), in the larval zebrafish. Limited field trial results of a heating oil additive package developed to minimize unscheduled maintenance indicate that it achieves its goal of keeping heating oil systems cleaner. The multifunctional additive package was developed to provide improved fuel oxidation stability, improved corrosion protection, and dispersency. This combination of performance benefits was chosen because we believed it would retard the formation of sludge, as well as allow sludge already present to be carried through the system without fouling the fuel system components (dispersency should keep sludge particles small so they pass through the filtering system). Wood plastic composite (WPC) formation was studied with simul wood and butylmethacrylate (BMA) at different compositions with swelling solvent methanol in the presence of a set of multifunctional acrylate monomer additives like N-vinyl Pyrrolidone (NVP), tripropylene glycol diacrylate (TPGDA) and trimethylol propane triacrylate (TMPTA) at a different doses between 1 and 3 Mrad using Co{sup 60} gamma source. This was further investigated using another set of additives chosen from oligomers such as epoxy acrylate (EA), urethane acrylate (UA) and polyester acrylate (PEA). TMPTA yielded the highest polymer loading at relatively low dose (2 Mrad). Effect of co-additives like sulfuric acid (H{sup +} ions), copper sulphate (Cu{sup 2+} ions) and urea was investigated in these series. Tensile strength (TS) of the composites were determined and the highest TS values were obtained with the composite produced in presence of urea. (author).

Besides the multifunctionality, another equally important aspect of nanoparticles is their engineerability to control the geometrical and chemical properties during fabrication. In this work, we exploited this aspect to define asymmetric surface chemistry of an iron oxide nanosphere by controlling the topology of ligand expression on its surface resulting in a particle with two faces, one displaying only amines and the other only thiols. Specifically, amine-functionalized iron oxide nanospheres were attached on a solid support via a crosslinker containing a disulfide bridge. Liberation of the nanosphere using thiolytic cleavage created thiols on the portion of the particle's surface that interacted with the solid support. Eliminating anterior tooth contact is paramount when treating combination syndrome. It is possible to do so through the use of linear occlusion with a non-interceptive arrangement and the bilateral fulcrum of protrusive stability. The presence of mandibular anterior teeth often complicates this procedure. When establishing the horizontal plane of occlusion, clinical circumstances often necessitate modifications; nevertheless, certain principles must be incorporated into the design of the prostheses. Interferons, a group of cytokines with antiangiogenic, direct antitumour and immunostimulating properties, have shown significant activity against osteosarcoma in vitro and in xenograft models. They have also been used in osteosarcoma clinical trials as a single adjuvant to surgery, with an apparent increase in relapse-free survival. In the ongoing EURAMOS 1 clinical trial, interferon a-2b is evaluated as an adjuvant treatment in osteosarcoma. This article reviews the rationale for the use of interferon in cancer with special reference to the treatment of osteosarcoma, including all published data of clinical efficacy in this disease. Clinical and hormonal indices of androgenic activity were compared in 118 males chronically intoxicated with alcohol. The population was divided into two groups with respect to their clinical and hormonal androgenic indices: 76 patients with (Group 1), and 46 patients without (Group 2) androgenic insufficiency. Distinct signs and syndromes of alcohol addiction were shown to correlate with the extent of cross-regulation impairment within hypothalamo-hypophyseo-gonadal system. This was particularly derived from comparison of clinical picture of chronic alcohol intoxication and hormone activity in the groups. Conventional therapy failed to restore the neuroendocrine indices that was apparently due to inhibition of cross-regulation links in the hypothalamo-hypophyseo-gonadal system at several stages of alcohol addiction. The diagnostic value of the portable chest radiography obtained with Fuji computed radiography (FCR) was evaluated in comparison with conventional images in 23 patients who were admitted to ICU in Yokohama City University Hospital. On the whole , FCR produced more exellent images than conventional screen-film radiographs. Especially the evaluation for the demonstration of the catheters, the endotracheal tubes and the tracheal shadows obtained high score. The conflict results mainly from insufficient cooling water heating with consumption-optimized engines. The usefulness and clinical applicability of computed radiography(FCR) in X-ray examination of the stomach were evaluated in comparison with film-screen (FS) radiography by both phantom experiments and clinical studies. Receiver operating characteristic (ROC) curve analysis consisting of 25 normal and 25 artificially created simulated lesions on an orginally created Styrofoam phantom showed no difference in lesion detectability between FCR and FS radiography. Both were also equal in evaluation of the internal structure and margin of the 25 lesions by visual ranking. ROC curve analysis of clinical cases was performed in 30 cases, 20 normal and 10 abnormal, while evaluation by visual ranking was performed on 18 lesions. We investigated 161 children under 24 months of age by CT of the skull and reviewed the anamnestic and clinical history. 87 children showed subdural effusions, and 74 did not. There were 33 patients with other pathological findings in CT, and 41 had normal scans. Age and sex distribution as well as localization of the subdural effusions were consistent with the literature. The 87 children with subdural effusions represented 17% in a sample of about 5000 CT scans. CT was the most reliable method for diagnosis of subdural effusions, compared to other techniques. There was a preponderance of small subdural effusions from 1 to 7 mm thickness (51%). Other CT abnormalities accompanying subdural effusions were found. Most frequently the interhemispheric sulcus was dilated and an internal hydrocephalus was present. Also in our group there were 7 anamnestical and 6 clinical symptoms highly diagnostic of subdural effusions.

Paraquat is a nonselective contact herbicide of great toxicological importance, being associated with high mortality rates, mainly due to respiratory failure. We report the case of a 22-year-old male admitted to the emergency room with a sore throat, dysphagia, hemoptysis, and retrosternal pain after the ingestion of 50 mL of a paraquat solution, four days prior to admission. Chest CT scans revealed pulmonary opacities, pneumomediastinum, pneumothorax, and subcutaneous emphysema. The patient was submitted to two cycles of immunosuppressive therapy with cyclophosphamide, methylprednisolone, and dexamethasone. The pulmonary gas exchange parameters gradually improved, and the patient was discharged four weeks later. The clinical and tomographic follow-up evaluations performed at four months after discharge showed that there had been further clinical improvement. Two patients who developed large intracranial vessel occlusion after standard radiation therapy for brain tumor are described. This form of vascular occlusion is usually seen in patients who have previously been treated by radiotherapy for intracranial tumor who then develop a relatively acute change in neurologic status. Histology of the lesion mimics accelerated focal arteriosclerosis. The clinical and radiographic manifestations of one case were highly atypical. The vasculopathy became evident shortly after termination of radiation therapy for a fourth ventricular ependymoma, and the angiographic picture stimulated a diffuse arteritis. The second patient was more typical, with clinical symptoms developing 12 years after radiation therapy for an oligodendroglioma. A recent development in meta-analysis, multiple treatment comparison (MTC) meta-analysis, provides inferences on the comparative effectiveness of interventions that may have never been directly evaluated in clinical trials. This new approach may be confusing for clinicians and methodologists and raises specific challenges relevant to certain areas of medicine. This article addresses the methodological concepts of MTC meta-analysis, including issues of heterogeneity, choice of model, and adequacy of sample sizes. We address domain-specific challenges relevant to disciplines of medicine, including baseline risks of patient populations. Anaphylaxis is a serious allergic reaction that is rapid in onset and sometimes leads to death. Understanding mechanisms, triggers, and patient-specific risk factors for severe or fatal anaphylaxis is critically important. Diagnosis of anaphylaxis is currently based on established clinical criteria. Epinephrine (adrenaline) is the first-line medication for anaphylaxis treatment and delay in injecting it contributes to biphasic reactions, hypoxic-ischemic encephalopathy, and fatality. The aim of this study was to evaluate typical magnetic resonance imaging (MRI) findings in early rheumatic diseases manifesting at the soft tissues of the hand using a retrospective analysis. A total of 186 MRI examinations of patients with clinical suspicion of a rheumatic disease were evaluated in a consensus reading by two experienced radiologists. All imaging patterns were assessed with respect to their type and localization. Under blinded and non-blinded conditions diagnoses were correlated with final clinical diagnosis. The most frequent diagnoses were rheumatoid arthritis (RA, 45.7%) and psoriatic arthritis (PsA, 15.6%). The mean correlation between clinical and MRI diagnosis (r) was 0.75 in blinded and 0.853 in non-blinded reading (p <0.001).
