Balloon dilatation and balloon-expandable stents for PTA of proximal venous stenoses in haemodialysis patients. Dilatation und ballonexpandierbare Stents zur Therapie zentralvenoeser Stenosen bei Dialysepatienten

On 10 dialysis patients we performed 12 balloon dilatations, 2 catheter lyses, 6 stent implants (Palmaz stent) and one atherectomy of central venous stenoses or occlusions (v. subclavia, v. brachiocephalica) at the shunt arm of the patient. The primary success rate was, in balloon PTA and lysis, 12/14 interventions, and in stent placement and atherectomy 7/7. The angiographical and clinical primary result after stent implantation was significantly better than after conventional dilatation. After 66% of the balloon dilatations recidivation occurred within the first year; this can be treated by means of repeated PTA. Whether long-term exclusion of recurrence can be achieved by stent implantation, must be established by means of follow-up studies that are at present in progress. (orig.).

LP steam turbine retrofits are generally undertaken as a result of mechanical problems, for example due to stress corrosion cracking, torsional vibration or erosion. They present plant operators with an opportunity to introduce advanced aerodynamic technology, and at the same time improve efficiency and availability. Standard components, the majority proven in long-term service, can be used to obtain a permanent solution to the problems. In several European countries, retrofits have been carried out on intact LP steam turbines owned by utilities whose primary motivation is efficiency improvement. (orig.) By joining the `climate association of European cities`, Frankfurt has agreed to reduce its CO{sub 2}-emissions by 50% by the year 2010. The so-called `climate offense 1991` was the first regulatory paper to be set up by a community to reach this goal over a long term. This report now represents the present level of the project. Main points are the development of houses with low entropy, the retrofitting of heating systems, water saving measures and energy management. The present publication is based on facts, data, knowledge and projects, which have been incorporated in the development of an energy concept for the district Schmalkalden-Meiningen. The energy concept of a district is a concept oriented at action on and realisation of mid-term and long term decisions in the fields of energy policy, economy and ecology, which must be taken by municipal and economic decision-making bodies. The energy concept accords with the targets of the Agenda 21, the environmental targets of the Federal Republic of Germany and of the Land Thuringia. The issue of projecting policies ensuring appropriate energy supply in the future has become more important in Germany after conclusion of the agreement on a long-term nuclear power phase-out. A recent comparative analysis of a selection of scientific studies forecasting the implications of a nuclear power phase-out has shown that there is demand for more studies examining the issue in greater detail, and on the basis of an enlarged scope of research topics. Hence the present study, intended as a preliminary analysis, will investigate the potential, conditions of successful application, conceivable support schemes, and effects of those policy options that in any case, apart from the nuclear power phase-out scenario, are of essential importance in the context of building the proper conditions for sustainable development of Germany's energy sector.
