Longitudinal changes in extended roles in radiography

author.The existing management of hospital waste in Lebanon currently poses both an environmental hazard as well as a public health risk. This is due mainly to lack of legislation, information and modern treatment and disposal facilities designed for this purpose. A nation-wide questionnaire survey was conducted to asses the status of hospital waste management. The study started from October 1997 till August 1998. We found that 75% of the surveyed hospitals completely ignore their total waste quantity: 73% of hospitals surveyed practice segregation at source of infectious, pathological, sharps and pharmaceuticals; more than 40% dispose of their hospital risk wastes through the municipality waste disposal, 24% by burning in open fires, 14% by on-site hospital incinerators, 11% in on-site dumping, 8% handled by a private contractor and 1% in ...

Purpose: The purpose of this research was to identify the extent and scope of changes to radiography practice. Method: Questionnaires were sent to radiology managers at acute National Health Service (NHS) hospitals in 2000 as a follow-up to an initial survey conducted in 1998. Information was sought on region, teaching/non-teaching status, the nature of extended role tasks undertaken and the year in which these tasks were first undertaken. Results: Some 172 questionnaires were returned from a total of 253 dispatched (68%). In 161 hospitals radiographers administered intravenous injections and performed barium enemas in 119 hospitals, while a red-dot system was in operation in 141 hospitals. The National Radiological Protection Board is to carry out a survey of doses received by patients from the diagnostic use of X-rays, beginning in June 1977, and carried out at over 100 National Health Service hospitals. Details are given of the reasons for carrying out the survey and the survey objectives. The survey working methods are discussed, together with methods of calculation of the genetically significant dose. Economic regulation and hospital behavior: the effects on medical staff organization and hospital-physician relationships.

On 21 October 1994, three brothers entered a waste repository at Tammiku, Estonia, without authorization and removed a metal container enclosing a caesium-137 source. During the removal the source was dislodged and fell to the ground. One of the men picked up the source, placed it in his pocket and took it to his home in the nearby village of Kiisa. Very soon after entry into the repository he began to feel ill, and few hours later he began to vomit. The man was subsequently admitted to hospital with severe injuries to his leg and hip and died on 2 November 1994. The injury and subsequent death were not attributed to radiation exposure, and the source remained in the man's house with his wife and stepson and the boy's great-grandmother. The boy was hospitalized on 17 November with severe burns on his hands, and these were identified by a doctor as radiation induced. The epidemiology of Graves' disease: Evidence of a genetic and an environmental contribution

PURPOSE: To investigate differences in cost of endovenous laser therapy (EVLT) using a top-down approach derived from the Annual Report versus a clinically orientated, bottom-up approach at a single hospital. METHODS: Information was obtained from: the day-case activity Service Line Report (SLR) income statement for general surgery, comparative data from the National Audit Commission, reference costs from the hospital finance department on 69 patients and calculations on individual treatment times and session slots (2 EVLT's per 3.5 hr session) on 37 consecutive patients. Duration of treatment, consumables (over #3) and staff pay were also recorded. Overheads were estimated at 15% and adjustments were made based on location and length of stay. Risk of malignant lymphoma in patients with inflammatory bowel diseases: A Dutch nationwide study.

Acute vertebrobasilar occlusion (VBO) is associated with a high risk of stroke and death. Although local thrombolysis may achieve recanalization and improve outcome, mortality is still between 35% and 75%. However, without recanalization the chance of a good outcome is extremely poor, with mortality rates of 80-90%. Early treatment is a fundamental factor, but detailed studies of the exact time management of the diagnostic and interventional workflow are still lacking. Long term indoor radon gas measurements using a passive monitor were conducted in the basement rooms of five hospitals, which are built in the Tokyo metropolitan area. The ventilation rate averaged over all hospital was 12.3 air change per hour (ACH). The highest radon concentration was 56.3 Bq/m"3. The value averaged concentration over all hospital and all season was 20 Bq/m"3. But the average of the two hospitals were twice as high as those of three others. Since the total ventilation rate integrated over a day is not different much, the reason of the concentration difference is considered to be the difference in radon emission rate from concrete wall and soil of the each hospital. Though we inquired workers about 'Environmental complaints', it do not show the direct relation between radon and environmental complaints. (author)

In this project, measurement of x-ray radiation dose level during radiological examination in Jos university teaching hospital and Plateau State specialist hospital at different locations within the department were carried out using radiation survey meter. The results were converted from micro sievert per hour (#mu#Sv/hr) to milisievert per year (mSv/yr). The radiation levels from the two hospitals visited ranged from 1.3m Sv/yr to 8.4m Sv/yr. This implies that all the radiology departments visited are still operating within the safety limit having less than 20 mSv/yr which is the standard permissible radiation level recommended per year.

Full text.National programs of neonatal screening were introduced in most countries and have proved their effectiveness against different types of retardation in children. Congenital hypothyroidism is one of the most common causes of physical and mental retardation in children. Programs of screening have shown that the incidence of this disorder is ethnic-background: it is low in Africans: 1-32000 whereas it is 1-3500 and 1-5000 in Hispanic and European newborns respectively. In clinical applications, it is very important to accurately determine the range of a proton beam in a patient. Recently, a prototype prompt-gamma scanning system, called a prompt-gamma scanner (PGS), has been constructed at Hanyang University in Korea with the collaboration of the Korean national cancer center (NCC), based on a series of Monte Carlo simulations. The PGS system is composed of a prompt-gamma collimator, a CsI(Tl) scintillation detector, a multi-channel analyzer (MCA), a precision movement system, and an integrated readout system. The prompt-gamma collimator is designed to effectively shield high-energy neutrons from the proton beam passage and subsequent capture gammas. The Nuclear Regulatory Commission (NRC) on December 29, 1993, promulgated self-guarantee requirements that materials licensees may use to demonstrate financial assurance for decommissioning costs. However, nonprofit colleges and universities, nonprofit hospitals, and for-profit firms that do not issue bonds are currently precluded, by their unique accounting and financial reporting systems, or by other features of their business practices, from using the financial tests for self-guarantors adopted by the NRC. This Report evaluates several alternative financial tests that might serve as the basis for self-guarantee by these three categories of licensees.

The study described in this report is part of a project sponsored by the Toronto Atmospheric Fund, performed at the Lawrence Berkeley National Laboratory, to assess the potential role of surface property modifications on energy, meteorology, and air quality in the Greater Toronto Area (GTA), Canada. Numerical models were used to establish the possible meteorological and ozone air-quality impacts of increased urban albedo and vegetative fraction, i.e., ''cool-city'' strategies that can mitigate the urban heat island (UHI), significantly reduce urban energy consumption, and improve thermal comfort, particularly during periods of hot weather in summer. Mitigation is even more important during critical heat wave periods with possible increased heat-related hospitalization and mortality. Background:The validity of dementia diagnoses in the Danish nationwide hospital registers was evaluated to determine the value of these registers in epidemiological research about dementia. Methods: Two hundred patients were randomly selected from 4,682 patients registered for the first time with a dementia diagnosis in the last 6 months of 2003. The patients' medical journals were reviewed to evaluate if they fulfilled ICD-10 and/or DSM-IV criteria for dementia and specific dementia subtypes. The patients who were still alive in 2006 were invited to an interview. Results: One hundred and ninety-seven journals were available for review and 51 patients were interviewed. A registered diagnosis of dementia was found to be correct in 169 (85.8%) cases. Regarding dementia subtypes, the degree of agreement between the registers and the results of the validating process was low with a kappa of 0.36 (95% CI 0.24-0.48). The incidence of acute kidney injury in patients with traumatic brain injury

To quantify the number of cases and clinical courses of Acanthamoeba keratitis treated at the King Khaled Eye Specialist Hospital (KKESH) between December 1982 and May 2005, and to review their clinical course and response to medical and surgical therapy. Retrospective review of medical records of all patients with Acanthamoeba keratitis diagnosed by the KKESH Microbiology or Pathology or Pathology Laboratory since the opening of the hospital. During a period of over 22 years, only six cases of confirmed. Acanthamoeba keratitis were diagnosedand treated at KKESH. An electronic literature search was made to identify English-language articles available through March 2011, addressing the effect of patient treatment volume on outcome for patients with subarachnoid hemorrhage. Limited data were identified, with 16 citations included in the current review. Over 60% of hospitals fall into the lowest case-volume quartile. A retrospective comparison of endoscopic stenting alone with stenting and radiotherapy in non-resectable cholangiocarcinoma

The authors report the personal techniques and the results of CT-guided percutaneous drainage of postoperative abdominal fluid collections. CT-guided percutaneous drainage offers many advantages over surgery: it is less invasive, can be repeated and requires no anesthesia, there are not surgery-related risks and lower morbidity and mortality rates. Moreover, subsequent hospitalization is shorter and costs are consequently reduced. The authors conclude that CT-guided percutaneous drainage is the method of choice in the treatment of postoperative abdominal fluid collections.

Radiotherapy has been reported to be of benefit in prolonging the survival of patients with cholangiocarcinoma. This study examined whether radiotherapy in addition to endoscopic stenting improved survival. The survival advantage of radiotherapy in those with a type II/III stricture is seen only in the first 10 months after diagnosis. Managing the Army National Guard Full-Time Force

The National Energy Modeling System (NEMS) developed by the U.S. Department of Energy`s Energy Information Administration is a well-recognized model that is used to project the potential impact of new electric generation technologies. The NEMS model does not presently have the capability to model energy storage on the national grid. The scope of this study was to assess the feasibility of, and make recommendations for, the modeling of battery energy storage systems in the Electricity Market of the NEMS. Physical aspects of quality assurance in radiotherapy: A protocol for quality control

The solid-pseudopapillary tumor (SPT) of the pancreas is a rare type of exocrine pancreatic neoplasm. SPT predominantly affects young women and female children, and is usually discovered incidentally. This tumor is generally benign with a low incidence of malignancy. A 13-year-old girl was admitted to the hospital with a few weeks' history of mild abdominal pain and jaundice. On physical examination, there was no palpable mass. The laboratory tests showed increased SR, CRP, high bilirubin, amylase and lipase serum levels. Ultrasound imaging revealed a solid lesion in the region of the pancreatic head. On MRI, precise tumor localization in the head of the pancreas with pancreatic duct dilatation and compression of the common bile duct were visualized. Pancreaticoduodenectomy and cholecystectomy was performed with good clinical outcome. Microscopic and immunohistochemical studies indicated that tumor cells were typical of SPT without any signs of malignancy. After ...

Epidemiologic studies of coronary heart disease and stroke in Japanese men living in Japan, Hawaii, and California: demographic, physical, dietary, and biochemical characteristics

An evaluation was made of possible hazardous working conditions at the ASARCO Globe facility, Denver, Colorado. The facility was involved in the recovery of cadmium from baghouse dust, a waste byproduct of nonferrous smelters. A medical study of kidney function was conducted in 45 current or former cadmium production workers; results were compared to those for 35 hospital workers. Kidney abnormalities were more common and more severe in the cadmium workers than in the hospital workers. Abnormalities of proximal tubular function included reduced reabsorption of beta-2-microglobulin, retinol binding protein, calcium and phosphate. Glomerular dysfunction was evidenced as elevated serum creatinine. As length of exposure to cadmium increased, changes became more apparent in workers. These summary descriptive data of ethnically similar cohorts of indigenous and migrant Japanese males have shown similarities or slight differences in characteristics wholly or largely genetically determined, such as blood groups, stature, and skeletal size. Differences have been noted in characteristics largely environmental or behavioral, such as diet and cigarette smoking habit, as well as in characteristics determined by a varying mixture of genetic and environmental influences, such as weight, blood pressure, glucose tolerance, and serum lipid and uric acid levels. The literature is littered with reports presenting the proportion of patients admitted with ischaemic stroke receiving thrombolysis. This gives some measure of activity and the efficiency of prehospital and hospital triage. Some stroke centres have reported rates of thrombolysis as high as 20%, but without knowing the population that such services serve, these proportions do not indicate how well or equitably the treatment is being delivered. Most papers appearing in the surgical literature dealing with wound ballistics concern themselves with wound management in the civilian setting. The pathophysiology of modern war wounds is contrasted with ballistic wounds commonly encountered in peacetime, but it should be noted that even in peacetime the modern terrorist may have access to sophisticated military weaponry, and that patients injured by them may fall within the catchment area of any civilian hospital. The path to the future: The role of science and technology at Argonne National Laboratory

To evaluate the diagnostic performance of commercially available CRT monitors used in a teleradiology system by determining the rate of detection of simulated lung nodules. Three types of CRT monitors were tested in the observation study. They had matrix sizes of 1024 x 768 (16 inches, color), 1024 x 768 (20 inches, color) and 1600 x 1125 (24 inches, black and white). Twenty chest radiographs were obtained by Fuji computed radiography (FCR) of an anthropomorphic chest phantom with ten simulated nodules on its surface. These FCR films were digitized by a film digitizer with 125 DPI (1024 x 1024 matrix sizes) and 12 bit gray scales, and the image data were transferred from Shinshu University Hospital to other hospitals where interpretation was carried out by the radiologists. Brief comments are given on the evolution of the institutional nature of ENEL (Italian Electricity Board) as it relates to the framework of legislation for the nationalization of the utility in December, 1962. Special attention is given to the development of legislative criteria to allow the creation of a rate structure, in the case of autoproduction by industry, that would stimulate national economic growth. A discussion is also made of the changing role of ENEL as it responds to the planning objectives cited in the Italian National Energy Plan which calls for a more integrated effort by the regional governments and the Electricity Board in the management of natural resources.

Stability and disturbance of large dc superconducting magnets

Many nations and international agencies are working to develop improved technology and industrial capability for neuclear fuel cycle and waste management operations. The effort in some countries is limited to research in university laboratories on treating low-level waste from reactor plant operations. In other countries, national nuclear research institutes are engaged in major programs in all phases of the fuel cycle and waste management, and there is a national effort to commercialize fuel cycle operations. Since late 1976, staff members of Pacific Northwest Laboratory have been working under US Department of Energy sponsorship to assemble and consolidate openly available information on foreign and international nuclear waste management programs and technology. This paper addresses the stability aspects of several successful dc superconducting magnets such as large bubble chamber magnets, and magnets for the Mirror Fusion Test Facility and MHD Research Facility. Specifically, it will cover Argonne National Laboratory 12-Foot Bubble Chamber magnets, the 15-foot Bubble Chamber magnets at Fermi National Laboratory, the MFTF-B Magnet System at Lawrence Livermore National Laboratory, the U-25B Bypass MHD Magnet, and the CFFF Superconducting MHD magnet built by Argonne National Laboratory. All of these magnets are cooled in pool-boiling mode. Magnet design is briefly reviewed. Discussed in detail are the adopted stability critera, analyses of stability and disturbance, stability simulation, and the final results of magnet performance and the observed coil disturbances.
