This study evaluated the clinical efficacy of treatment with oral risedronate (17.5?mg once daily) for 8?weeks in 11 Japanese patients with Paget?s disease of bone (PDB). Risedronate suppressed the excessive bone turnover associated with PDB and improved several biochemical markers, including serum alkaline phosphatase (ALP), serum bone-specific ALP (BALP), urinary deoxypyridinoline (DPD), and urinary cross-linked N-telopeptide of type 1 collagen (NTX). For the purpose of nationwide surveillance of the antimicrobial susceptibility of bacterial respiratory pathogens collected from patients in Japan, the Japanese Society of Chemotherapy conducted a third year of nationwide surveillance during the period from January to April 2008. A total of 1,097 strains were collected from clinical specimens obtained from well-diagnosed adult patients with respiratory tract infections. A method is provided for detecting pathological macromolecules in a patient. The method is comprised of the following: obtaining body fluid from the patient; pretreating the body fluid; subjecting the pretreated body fluid to size-exclusion chromatography to create an excluded fluid; and analyzing the excluded fluid to detect macromolecules having a predetermined molecular weight. The method also allows for comparing elution spectra with reference spectra of suspect pathologic proteins.

Cockayne syndrome (CS) is a rare autosomal recessive disease, which shows diverse clinical symptoms such as photosensitivity, severe mental retardation and developmental defects. CS cells are hypersensitive to killing by ultraviolet (UV)-irradiation and defective in transcription-coupled repair. Two genetic complementation groups in CS (CS-A and CS-B) have been identified. We analyzed mutations of the CSA gene in 5 CS-A patients and identified 3 types of mutations. Four unrelated CS-A patients (CS2OS, CS2AW, Nps2 and CS2SE) had a deletion including exon 4, suggesting that there is a founder effect on the CSA mutation in Japanese CS-A patients. Patient CS2SE was a compound heterozygote for this deletion and an amino acid substitution at the 106th glutamine to proline (Q106P) in the WD-40 repeat motif of the CSA protein, which resulted in a defective nucleotide excision repair. ...

Rectum reference dose points have a direct impact on the morbidity seen in gynecological intracavitaty radiotherapy. The aim of this study is to obtain less morbidity by marking rectum with radio opaque solution. Fourteen patients with gynecological cancer who underwent external radiotherapy received 15 Gray (Gy) intracavitary radiotherapy. During the internal radiotherapy planning, radio opaque solution has been administered into the rectum to observe it on lateral radiogram. Thus rectum reference dose point and five points were marked at sites nearest to the sources. Nucletron Planning system was used for obtaining three-dimensional planning. The intracellular A chromogranin (ACg) is specific in neuroendocrine cells. This work is a contribution to the study of clinical interest of serumal dosage of this protein in a series of patients carrying neuroendocrine tumors (NET). From January `94 to November `96, 143 patients were subjected to OctreoScan scintigraphy and/or to MIBG. In 104 of them a dosage of ACg by RIA method was effectuated. A study of a statistical relation was performed on the rate of ACg and histopathological, clinical and scintigraphic criteria. Analysis of results excluded the patients with abnormal creatininemy; it referred exclusively to the patients the histopathology of whose it was proved. To explain the meaning and to trace the evolution of scientific truth, this article outlines the history of the Scientific Revolution and of the parable of Modernity, up to the arrival of pragmatism and hermeneutics. Here, the concept of truth becomes somehow discomfiting and the momentum leans towards the integration of different points of view. The fuzzy set theory for the definition of disease, as well as the shift from disease to syndrome (which has operational relevance for geriatrics), seems to refer to a more complex perspective on knowledge, albeit one that is less defined as compared to the nosology in use. Pain is necessary for survival but chronic pain is disabling and causes significant health and economic problems. This study provides an understanding of the future for spinal cord stimulation. Stimulation by means of chronically implanted electrodes, was carried out in 200 patients with pain of varied benign organic etiology. In 177 of them, pain was confined to the failed back syndrome. Most patients were referred by a Pain Management Service. 226 epidural implants were used: 80 unipolar, 59 Resume, 12 bipolar, and 75 quadripolar. Patients were followed for periods of 6 months to 12 years, with a mean follow-up of 44 months. 84 patients (42%) were able to control their pain by stimulation alone, 22 patients (11%) needed occasional analgesic supplements along with their stimulation program. The purpose of this study is to develop a geometrically accurate imaging protocol at 3 T magnetic resonance imaging (MRI) for stereotactic radiosurgery (SRS) treatment planning. In order to achieve this purpose, a methodology is developed to investigate the geometric accuracy and stability of 3 T MRI for SRS in phantom and patient evaluations. Forty patients were enrolled on a prospective clinical trial. After frame placement prior to SRS, each patient underwent 3 T MRI after 1.5 T MRI and CT. MR imaging protocols included a T1-weighted gradient echo sequence and a T2-weighted spin echo sequence. A rare case of primary splenic cyst is shown in a young woman who had a left subcostal abdominal pain. Abdominal echography and CT scan revealed a cyst of the anterior aspect of the spleen. A sorologic test for hidatic disease was negative. On the basis of a presumed diagnostic of nonparasitic cyst, the patient was referred to a laparoscopic decapsulation with excision of the cysts wall not covered by splenic tissue. The patient was discharged 24 hours later. Histological report revealed epidermoid cyst. The laparoscopic approach has being recently considered an effective and less invasive alternative in the treatment of splenic diseases. We demonstrated that it should be considered for the treatment of splenic cysts present in a superficial location, with the advantage of organ preservation. (author)

Contributions to the population dose due to medical applications of ionising radiation are presented. It appears that contributions due to radiotherapy and X ray diagnosis are of comparable magnitude. To account for the age dependency of the radiation risks, contributions in terms of somatically significant dose equivalent are calculated as 0.3 mSv per caput per annum for X ray diagnosis and as 0.2 mSv per caput per annum for radiotherapy, the latter referring to only doses to organs outside the target volume. We encountered a case of Ewing's sarcoma. The patient was a 19 year-old girl, who visited a local physician because of pain in the left scapular region, and was referred to us. Roentgenograms of her left scapula revealed an irregular moth-eaten shadow which was suspected to be a malignant bone tumor. The histological diagnosis of Ewing's sarcoma was comfirmed by needle biopsy. The initial stage of treatment consisted of radiation therapy (6000 rads) and chemotherapy (VAC and ADR). At the end of the initial stage, the symptoms were improved, the size of the local lesion was reduced on the roentgenogram and there was no evidence of distant metastases. (author).

Soil dressing on coal ash reclaimed land is a covering to use the land for agriculture. This study was carried out to find out the necessary depth of soil covering the ash layer in order to have normal crop growth. Two kinds of crops, Japanese radish, and rakkyo were planted in cover soil on the fly ash packed in wooden boxe (90 cm x 90 cm x 90 cm). Depths of cover soil were 20 cm, 30 cm, 40 cm and 50 cm. The results were summarized as follows: growth and yield of Japanese radish and rakkyo were increased with increasing depth of cover soil; root development of Japanese radish was inhibited at the lower coal ash layer. Main root length and edible root length of Japanese radish were decreased with decreasing depth of cover soil; boron and molybdenum contents in the plants remarkably increased with decreasing depth of cover soil. Introduction: Thalassemia is the most common hereditary blood disorder in the world. Iran is located on the thalassemic belt and there is a high prevalence of the hepatosplenomegaly in beta thalassemia minor patients which is reported to be very variable. The goal of this research was to study the frequency of these signs in the cases with beta thalassemia minor patients in Iran. Materials and methods: Two hundred and fifty-nine cases that referred to center for pre-marriage tests were divided into two groups according to their MCV, MCH, and HbA2 (beta thalassemia minor cases and control groups). Computerized Tomography has gained an important role in the diagnosis of the complications of prosthetic surgery of the abdominal aorta: the importance of such complications come from their frequency, which is proportional to the increasing number of interventions, and their severity. The authors investigated the CT patterns of the most frequent complications. 24 patients referred for strongly suspected postoperative complications were examined in 2 years: fever and leukocytosis (20 cases) and progressive anemia (4 cases) were the most frequent findings. The patients studied were referred for routine abscess localization with indium-111 labelled autologous leukocytes. One series (n=15) was studied at 1 day after reinjection of labelled cells and a second series (n=9) was studied between 4 hours and 4 days after reinjection of labelled cells. The leukocytes were isolated by a gravity sedimentation method and labelled with In-111 oxinate according to Roevekamp and co-workers. The labelled cell suspension administered to the patient contained between 11 and 15 MegaBq (300-400 microCi). The indium-111 concentration in the liver and spleen was quantitated using a scintillation camera and an on-line computer system. After having discussed the peculiarities of children in X-ray diagnosis the application of the effective as well as the mean equivalent doses as to the determination of the radiation exposure in children is represented. In using computerized tomograms to determine position and extent of an organ the exposure by repeated X-raying was calculated referring to the entrance dose. Entrance dose measurements for all types of X-ray examination in hospitals and ascertainments of the frequency of radiograms in 20 X-ray departments were used to determine per capita and collective doses, resp., in several age groups. An investigation of the effect of blending of two different coals in various ratios on the hydrogenation reaction was carried out using a red mud-sulphur catalyst. Three Japanese coals and an Australian brown coal were used in the study. The conversion of the blended coals was compared with that obtained with each of the coals alone. A blend of the Yallourn brown coal and Shin-Yubari coal showed a synergism: it is considered that hydroaromatics from the Japanese coal promote the hydrogenation of the Yallourn coal. On the basis of observations of the JTEC team, it appears that Japanese universities and research institutes are leading long-term R&D thrusts for development of new materials casting technologies. Significant efforts include amorphous metals, intermetallics, application of MHD in continuous casting of steel, and energy efficient furnace technology. Industrial R&D seems focused more on process improvements than on new product technologies, but significant efforts in new cast materials included cast metal matrix composites, materials substitutions for thinner wall products, and advanced ceramic products for foundry industry applications.

Radiation therapy has had a significant and an expanded role in the management of cancer of the pancreas during the last decade. In particular, for locally advanced disease, radiation therapy has improved the median survival of patients to 1 year. Intraoperative electron beam therapy has been applied to unresectable and resectable pancreatic cancer in an attempt to enhance local control of disease and to improve patient survival. This paper presents a survey of the role of radiation therapy in treatment of cancer of the pancreas, provides information on the radiobiological aspects of this treatment modality and details the physical and dosimetric characteristics of intraoperative radiation therapy with electrons. In this work, thermoluminescent dosimeters have been used to measure entrance surface doses (ESDs) of patients undergoing mammographic screening at the University College Hospital, Ibadan, Oyo state. The mean glandular doses (MGDs) were also calculated using the measured ESDs. The results showed that the ESDs ranged from 0.26 mGy to 21.26 mGy for the mediolateral oblique (MLO) views and 0.08 mGy to 5.36 mGy for the craniocaudal (CC) views. The calculated MGD ranged from 0.07 mGy to 3.57 mGy for the MLO views and 0.02 mGy to 0.98 mGy for the CC views. The possible reasons for the large variations in the individual ESD values and MGD values for both views are discussed using patients' data, equipment specific data and the technical parameters used for the examinations. We wanted to assess the usefulness of four-dimensional (4D) ultrasonography (US), i.e., real-time three-dimensional US, as an adjunct for performing various US-guided interventional procedures in superficial lesion. Thirty-three patients were referred for US-guided interventional procedures for superficial lesions, including core biopsy in 19, fine- needle aspriation in eight, therapeutic drug injection in four and needle puncture in two. The procedures were performed under 4D US guidance. We reviewed the pathologic/cytologic results of the core biopsies or needle aspirations, and also the outcomes of drug injection or needle puncture. PURPOSE: To investigate differences in cost of endovenous laser therapy (EVLT) using a top-down approach derived from the Annual Report versus a clinically orientated, bottom-up approach at a single hospital. METHODS: Information was obtained from: the day-case activity Service Line Report (SLR) income statement for general surgery, comparative data from the National Audit Commission, reference costs from the hospital finance department on 69 patients and calculations on individual treatment times and session slots (2 EVLT's per 3.5 hr session) on 37 consecutive patients. Duration of treatment, consumables (over #3) and staff pay were also recorded. Overheads were estimated at 15% and adjustments were made based on location and length of stay. The paediatric radiology unit of the Inselspital Bern recently installed some of the most advanced X ray equipment nowadays available. It is based on the two latest digital technologies: double read computed radiography (CR) and direct digital radiography (DR). Hystreosalpingogram of 375 patients with infertility were reviewed with reference to radiological feature of each pathology and a comparative study of hysterosalpingography and laparoscopy in 36 patients was presented. This results were as follows;1. In 375 cases of infertility, 191 cases (50.9%) were primary infertility and 181 cases (49.1%) were secondary infertility. 2. The peak age distribution was 26 to 30 year old group in 211 cases (56.3%), and the peak duration of infertility was less than 3 years in 256 cases (68.3%). 3. Abnormal uterine findings were 115 cases (30.7%) and there were 73 cases of filling defects, 13 cases of irregularity, 12 cases of uterine flexion, 11 cases of congenital anomaly, and 9 cases of hypoplasia among them. 4. This study looks at how education policies in colonial Korea changed over time in order to accommodate the needs of the colonial authorities during the period of Japanese colonial rule in Korea (1910-1945). The colonial experience can be divided into four different periods according to the four Educational Ordinances issued in 1911, 1922, 1938, and 1943 by the colonial government with each period corresponding to a historic event or context. The constitutive relevance of colonial discourse for an understanding of education can be found in the gradual transformation of the education sector in Korea under colonial rule. It is evident that Japanese colonial policy was inherently contradictory in principle and in practice. On the one hand it sought the assimilation of the Koreans, while on the other it maintained its discriminatory and exploitative practices. Abstract: The current financial crisis has now led most major central banks to rely on quantitative easing. The unique Japanese experience of quantitative easing is the only experience which enables us to judge this therapy's effectiveness and the timing of the exit strategy. In this paper, we provide a new empirical framework to examine the effectiveness of Japanese monetary policy during the ''lost'' decade and quantify the effect of quantitative easing on Japan's activity and prices. The BASH-M code, developed for large scale LOCA reflooding analysis, is given as an example of verification and improvement of US computer programs are given. The code's application to the operational safety analysis concerns the following main areas: 1D drift flux model base computer program CANAC; CANAC-based advanced training simulator; emergency operating procedures. The authors report an effort to determine the basis for the altered migration of seven inherited albumin variants detected by one-dimensional electrophoresis in population surveys involving tribal Amerindians and Japanese children. An amino acid substitution has thus far been determined for four of the variants. The randomness in the albumin polypeptide of these and the other sixteen independently ascertained amino acid substitutions of albumin and proalbumin thus far established was analyzed; the clustering of eight of these at two positions in the six-amino acid propeptide sequence seems noteworthy. By comparison with other proteins studied by electrophoresis, albumin exhibits average variability. It is a paradox that individuals who, for genetic reasons, lack albumin exhibit no obvious ill effects; yet, electrophoretic variants of albumin are no more numerous than are variants of proteins, the absence of which results in severe disease.

Mycobacterium xenopi is one of the most common agents responsible for nontubercolar mycobacterial pulmonary disease on AIDS patients. These lesions have been studied with conventional radiography while CT has been used in patients with aspecific mycobacterioses or non-AIDS pulmonary conditions from Mycobacterium xenopi. 12 AIDS patients were examined. They had pulmonary lesions from Mycobacterium xenopi, patients age ranged 30 to 46 years. All patients had CD4 blood levels lower than 250 cells/mL and Mycobacterium xenopi in the sputum. All patients underwent a standard chest radiograph and a CT examination. CT images were evaluated by three radiologists independently and the definitive diagnosis was made in the presence of a fourth radiologist. A new microdensitometry method for quantitative assessment of bone mineral content using Fuji computed radiography (FCR) was developed and the results were compared with those obtained by the conventional film-screen method. Using both methods, X-ray pictures of the 2nd metacarpal bone were taken with a reference aluminum step wedge in nine normal subjects and nine parameters representing the width and density of bone marrow and cortex were calculated in middle of the bone shaft. Interobserver reproducibility assessed for three parameters was significantly higher in the FCR method than the conventional one. The value of #SIGMA#GS computed by the FCR method, that is equivalent to the net bone density of the transverse section of the shaft, showed higher correlation with the one obtained by computed tomography in comparison with the conventional method. For the step-and-shoot intensity-modulated radiation therapy (IMRT) technique, the combination of high dose rate, multiple beam segments and low dose per segment can lead to significant differences between the planned dose and the dose delivered to the patient. In this technique, a dose delivery inaccuracy known as the 'overshoot' effect is caused by the dose servo control system. This typically occurs in the first and last beam segments and causes an over- and underdose, respectively. Some dose positional error in the segment sequence is also possible there. The absorbed dose delivered to the patient in brachytherapy is directly proportional to the source strength in terms of the reference air-kerma rate (RAKR). Verification of this quantity by the hospitals is widely recognized as an important part of a quality assurance program. An external audit was performed on behalf of the Secondary Standard Dosimetry Laboratory at the Swedish Radiation Protection Authority (SSI). The aim was to investigate how accurately the source-strength in 192Ir brachytherapy is determined at Swedish hospitals. The SSI reference well-type ion chamber and calibrated equipment were used to measure the RAKR of an 192Ir source in each of the 14 Swedish afterloading units. By improving the currently used lactoperoxidase method, we were able to obtain radioiodinated chicken luteinizing hormone (LH) that shows high specific binding and low nonspecific binding to a crude plasma membrane fraction of testicular cells of the domestic fowl and the Japanese quail, and to the ovarian granulosa cells of the Japanese quail. The change we made from the original method consisted of (1) using chicken LH for radioiodination that was not only highly purified but also retained a high receptor binding potency; (2) controlling the level of incorporation of radioiodine into chicken LH molecules by employing a short reaction time and low temperature; and (3) fractionating radioiodinated chicken LH further by gel filtration using high-performance liquid chromatography. The origin of natural gas in the Japanese oil and gas fields are geochemically studied. Samples are taken from structural natural gas, surface gas seepage, water-dissolved natural gas and coal-field gas of the Japan Sea coast area. The origins of primary hydrocarbons are classified into bacteria gas and thermogenic gas, the latter being subclassified into gas in oil production zone with the per million deviation of the carbon isotope (/sup 13/C/sup 1/) of methane less than -35 and those gases with maturity higher than this. Surface gas seepage is subjected to migration and bacterial oxidation. Coal gas is similar to oil gas. The CO/sub 2/ concentration is 0 - 2% and /sup 13/CO/sub 2/ deviations from -30 - +30. As the first step for development of the corrosion resistant superheater tube materials of 500 C, 100 ata used in high efficient waste-to-energy plants, field corrosion tests of six conventional alloys were carried out at metal temperatures of 450 C and 550 C for 700 and 3,000 hours in four typical Japanese waste incineration plants. The test results indicate that austenitic alloys containing approximately 80 wt% [Cr+Ni] show excellent corrosion resistance. When the corrosive environment is severe, intergranular corrosion of 40{approximately}200 {micro}m depth occurs in stainless steel and high alloyed materials. It is confirmed quantitatively that corrosion behavior is influenced by environmental corrosion factors such as Cl concentration and thickness of deposits on tube surface, metal temperature, and flue gas temperature. A centralized, automatic monitoring system for coal mines has been developed. The system consists of newly-developed and improved sensors, transmission equipment, a minicomputer designed for scientific and technical calculations, and accompanying software (MOLES). MOLES performs six functions: input, computation, decision, memory, output and correction. Previously, centralized monitoring has been limited to single dimensional monitoring of the 'time and point' type. In Japan, cosmetics claiming hormesis effect are available through Internet. Although these cosmetics show the contents, they never mention the minor elements and radioactive sources. The existence of radioisotopes, however, was observed by measurements of the gamma-rays with a HPGe detector. In this study, in order to clarify the contents of trace elements, the hormesis cosmetics including radioactive sources were analyzed using INAA, PGAA and NAA with multiple gamma-ray detection (NAAMG). Nineteen elements were analyzed quantitatively in hormesis cosmetics by INAA, PGAA and NAAMG and 16 elements were detected qualitatively by SEM-EPMA. (author)

Epoxy-resin based neutron shielding material, NS-4-FR, is used for spent fuel transport and/or storage cask. In this paper the outline of thermal aging test performed to evaluate the heating effect on this neutron shielding material, NS-4-FR, is introduced. The test is consisted of two kinds of thermal aging test, one is 'Basic Test' and the other is 'Block Heating Test'. The prompt neutron spectra of {sup 241}Am, {sup 242m}Am, and {sup 243}Am were evaluated for the next version of Japanese Evaluated Nuclear Data Library, using the methodology developed by the authors. The method is based on the Madland-Nix theory with some improvements to consider the multimodal nature of the fission process, and shell effects on the level density parameters and the neutron multiplicity from light and heavy fragments. Summary.? Measuring von Willebrand factor (VWF) activity is essential to the diagnosis of von Willebrand disease (VWD). The VWF activity is usually assessed based on measurement of the ristocetin cofactor (VWF:RCo). However, that test is technically challenging and has high intra- and inter-assay variabilities. The HemosIL VWF activity (VWF:AC) is a fully automated assay, recently proposed as a good alternative to VWF:RCo for VWD diagnosis. This study was undertaken to assess this new method. First, the analytical performance of VWF:AC on an automated coagulo-meter (ACLTop) was determined, and then this new method was compared with VWF:RCo and the platelet function analyzer (PFA100) for 160 patients referred for VWD screening. The VWF:AC achieved acceptable precision with within-run and between-run coefficients of variation ranging from 2.3% to 14.1%, and linearity from 10% to 100%. In not infected knee prostheses bone scintigraphy is a possible method to diagnose mechanical loosening, and therefore, to affect treatment regimes in symptomatic patients. However, hitherto studies showed controversial results for the reliability of bone scintigraphy in diagnosing loosened knee prostheses by using asymptomatic control groups. Therefore, the aim of our study was to optimize the interpretation procedure and to evaluate the accuracy using results from revision surgery as standard. Methods: Retrospectively, we were able to examine the tibial component in 31 cemented prostheses. In this prostheses infection was excluded by histological or bacteriological examination during revision surgery. Carpal tunnel syndrome (CTS) is a common disorder, for which various conservative treatment options are available. The objective of this study is to determine the efficacy of the various conservative treatment options for relieving the symptoms of CTS. Computer-aided searches of MEDLINE (1/1966 to 3/2000), EMBASE (1/1988 to 2/2000) and the Cochrane Controlled Trials Register (2000, issue 1) were conducted, together with reference checking. Included were randomised controlled trials evaluating the efficacy of conservative treatment options in a study population of CTS patients, with a full report published in English, German, French or Dutch. Automatic segmentation of anatomical structures in medical images is a valuable tool for efficient computer-aided radiotherapy and surgery planning and an enabling technology for dynamic adaptive radiotherapy. This paper presents the design, algorithms and validation of new software for the automatic segmentation of CT images used for radiotherapy treatment planning. A coarse to fine approach is followed that consists of presegmentation, anatomic orientation and structure segmentation. No user input or a priori information about the image content is required. In presegmentation, the body outline, the bones and lung equivalent tissue are detected. Forty-three patients with basal and squamous cell carcinomata arising on the pinna were treated radically using a 10 MeV electron beam. The technique and dosage are described and discussed. Primary cancer control with retention of the pinna was achieved in 34 patients. Salvage pinnectomy was performed in four patients for recurrence and one patient for radiation necrosis. Two patients with large primary tumours failed to resolve and died of their disease. The advantages for the patient of the policy of primary radical electron beam therapy are discussed.

Two experiments used the McGill Pain Questionnaire (MPQ) to examine the affective dimension of pain in patients whose pain was secondary to malignancy. In experiment I, segregating groups of cancer patients on the basis of extreme scores (high versus low) on the MPQ failed to produce segregation on independent measures of affect and infirmity. This outcome contrasts with earlier work with chronic benign pain patients. Experiment II compared cancer pain patients matched with benign pain patients on intensity of pain report on the affective dimension of the MPQ. Cancer pain patients reported a reliably higher affective loading to their pain. Purpose: To evaluate the significance of preoperative localization of abnormal parathyroid glands to the surgical outcome in patients with primary hyperparathyroidism. Material and Methods: Thirty-nine patients with primary hyperparathyroidism were studied preoperatively with US (39 patients), CT (30 patients) and MR imaging (18 patients). The overall diagnostic accuracy for US was 87%, CT 66% and MR 94%. In patients with a single parathyroid adenoma US was the most cost-effective localization technique with a detection rate of 96%. CT had a lower detection rate (78%) but was of particular value for fairly large ectopic adenomas in the root of the neck. MR imaging was a good confirmatory test (93%). Therapeutic effect of p-aminobenzoic acid-N-xyloside Na (K-247) were studied. Eleven patients with a variety of solid tumors were treated with K-247 alone. K-247 was given orally 800 mg daily for 4 weeks. As for side effect of the drug, only mild gastritis was observed in a few patients. Partial response (over 25% reduction of tumor size) with a median duration of two months was observed in 3 patients. These cases were metastatic tumor of lung from the carcinoma of thyroid, metastatic tumors of lung from the carcinoma of kidney, and mediastinal tumor. This report deals with the preliminary results of trials in the DATECA project with stage I, II and III patients with non-seminomatous germ cell tumours. Stage 1 patients were randomized between infradiaphragmatic irradiation and observation. No significant difference in recurrence rates has been observed as yet. Eighteen of 95 patients had recurrence with a median time to relapse of 3 months. Fifteen patients achieved complete remission after treatment by combiantion chemotherapy while 3 patients are still undergoing treatment. Stage II patients received 6 series of cis-platinum, bleomycin, and vinblastine. The patients were initially randomized to receive chemotherapy alone versus chemotherapy plus irradiation. The RTA dual-fuel engine was developed in close collaboration between Sulzer Brothers Limited, Switzerland and its Japanese licensees Ishikawajima-Harima Heavy Industries Co. Ltd. (IHI), Nippon Kokan KK (NKK) and Sumitomo Heavy Industries Ltd. (SHI). The objective of this paper is to give an overview on the seismic evaluation and retrofitting procedures of reinforced concrete buildings within JICA technical cooperation project in Romania. The content of the paper covers a) an outline of the seismic evaluation; history and comparison of Romanian seismic design codes with the Japanese seismic evaluation guidelines, b) an outline of the retrofitting techniques which were transferred from Japan to Romania and structural tests for retrofitting techniques employed in Romania and c) retrofitting details that were used by JICA/NCSRR in the retrofitting design of two vulnerable buildings in Bucharest. The above-mentioned retrofitting projects are now under development of detailed design and therefore, in the near future, refining and improvement of solutions will be performed. (authors)

The funding for a coal-liquefaction demonstration plant near Morgantown, W.Va., appears more certain since Japan has committed $175 million toward the project. West Germany has already signed a memorandum of understanding to contribute 25% of the expected $700 million cost. West Virginia Governor J. Rockefeller has said that he feels sure the Carter Administration will approve the project since half the money is assured. The solvent-refined-coal (SRC) demonstration program began in July 1978 with the award of two separate multiphased contracts for development of a solid process, SRC-I, and a liquid process, SRC-II. First, it is mentioned that the conventional geoid-based concept of 'gravity anomaly' should be changed to a new definition, according to the GPS-determined ellipsoidal height system. Secondly, the results of many experiments, which have been made to monitor ocean-plate motions relative to the Japanese island-arcs, demonstrate that GPS is a powerful tool for obtaining temporal changes in horizontal displacement induced by the plate motion. Therefore, it is suggested that terrestrial measurements, such as triangulation and long-range leveling, would be replaced by GPS measurements in near future. We have conducted an agent-based simulation of chain bankruptcy. The propagation of credit risk on a network, i.e., chain bankruptcy, is the key to nderstanding largesized bankruptcies. In our model, decrease of revenue by the loss of accounts payable is modeled by an interaction term, and bankruptcy is defined as a capital deficit. Model parameters were estimated using financial data for 1,077 listed Japanese firms. Simulations of chain bankruptcy on the real transaction network consisting of those 1,077 firms were made with the estimated model parameters. Given an initial bankrupt firm, a list of chain bankrupt firms was obtained. This model can be used to detect high-risk links in a transaction network, for the management of chain bankruptcy.

A destructive spondyloarthropathy is reported in four patients undergoing maintenance hemodialysis for chronic renal disease. In a separate investigation a controlled, prospective radiographic study of the cervical spine revealed this spondyloarthropathy in 4 (15%) of 26 long-term dialysis patients. A single disk level was involved in three patients, and two disc levels were involved in one patient. This spondyloarthropathy correlated with the duration of dialysis but not with the radiographic evidence of renal osteodystrophy or severity of laboratory abnormalities associated with hyperparathyroidism. Somatostatin receptor expression has been demonstrated on a number of plasma cell lines. Therefore, we questioned whether somatostatin receptor scintigraphy (SRS) can be used to demonstrate in vivo multiple myeloma (MM) activity. SRS was performed in newly diagnosed (n = 9) or relapsing (n = 18) MM patients or in patients with localized plasmacytoma (n = 2). The results were compared with radiographic findings. A positive SRS was demonstrated in 44% of the newly diagnosed patients, in 83% of the relapsed patients and in both patients with plasmacytoma. The SRS findings corresponded with radiographic abnormalities in 40% of the patients. However, in relapsed patients 60% demonstrated increased SRS uptake in areas without new radiographic abnormalities. Sampling and Methods: In a cross-sectional study, 201 allergy workup patients were interviewed using the Structured Clinical Interview for DSM-IV; they answered a set of self-report questionnaires including the Schizotypal Personality Questionnaire subscale for MT and the Patient Health Questionnaire (PHQ). Intraspinal synovial cysts were diagnosed in six patients during a 5-year period and retrospectively studied. Plain films of the lumbar spine showed degenerative changes in all patients. Lumbar myelography showed a posterior and lateral defect caused by extradural compression. Using CT without contrast enhancement established the diagnosis in five of the six patients. In four cases MRI was performed, three before and after injection of gadolinium diethyline-triamine penta-acetic acid (Gd-DTPA). Including the patient with a false-negative CT, MRI was positive in all patients. (orig.)

The verification test program on zinc injection applicability to Japanese BWRs was started in 1997. Laboratory tests using high temperature water loops under BWR reactor water conditions are in progress. This paper is an interim report on results obtained so far. Co-58 and Zn-65 were simultaneously used in the Co radioactivity buildup test to evaluate zinc injection suppression effects towards cobalt deposition on pre-oxidized stainless steel. The following results were obtained. The Co deposition was suppressed effectively by Zn injection, even when there was a pre-oxide film. For the test piping that had the pre-oxide film formed under the NWC (normal water chemistry) condition, when soaked under the HWC (hydrogen water chemistry) condition a large amount of Co-58 was taken into a small part of the inner layer. The aim of this paper is to analyse the role that the Japanese Government has played in the development of alternatives to conventional vehicles, the effect of government programmes, and the importance of technical flexibility in government support schemes. The focus is on battery-powered electric vehicles (BPEVs), hybrid electric vehicles (HEVs), and fuel cell electric vehicles. The effects of government policy and the process of innovation are analysed from a systems approach drawing on the literature regarding technical change and innovation. The whole chain of government support, including the context in which these different policies have been implemented since the early 1970s, is studied.
