Therapeutic decisions are quite clear-cut for asymptomatic gallstone disease and acute cholecystitis. However, the appropriate therapeutic course for older patients with chronic cholecystitis may be less obvious. Watchful waiting may be reasonable for patients with mild and infrequent symptoms. For healthy patients, cholecystectomy is recommended if symptoms are becoming more frequent and severe. Laparoscopy may reduce the complication rate and be safely performed even in those with underlying medical illness. Oral dissolution therapy can be attempted for qualifying symptomatic patients who are at poor surgical risk or who refuse surgery. Shock wave lithotripsy and contact dissolution therapy show some promise but are currently experimental. PMID:8339941

A population of male civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease. Field activities started in December, 1982 and were concluded in July, 1984. Diagnosis was made using real-time ultrasonography. Participation in the study was 71.5%. Prevalence of gallstone disease was 8.2% and increased with age from 2.3% in the 20- to 25-year-old age group to 14.4% in the 60- to 69-year-old age group, based on both presence of gallstones and history of cholecystectomy. About one-third of the subjects with gallstone disease had previously been submitted to cholecystectomy. Only 7.7% of the subjects with presence of gallstones complained of at least one episode of biliary pain in the preceding 5 years. Frequency of "minor" dyspeptic symptoms was not different between men with and those without gallstones. PMID:3391519

Studies that estimate deficit probabilities in hydrothermal systems have generally ignored the response of demand to changing prices, in the belief that such response is largely irrelevant. We show that ignoring the response of demand to prices can lead to substantial over or under estimation of the probability of an energy deficit. To make our point we present an estimation of deficit probabilities in Chile's Central Interconnected System between 2006 and 2010. This period is characterized by tight supply, fast consumption growth and rising electricity prices. When the response of demand to rising prices is acknowledged, forecasted deficit probabilities and marginal costs are shown to be substantially lower. (author)

The consequences of exit, voice, loyalty, and neglect behavior were examined using retrospective questionnaires (Study 1) and 2 social interaction diary studies (Study 2). Exit and neglect were generally associated with more negative partner responses and reduced feelings of value and closeness, and were reported to be more harmful to the relationship. Voice was associated with more positive partner responses and greater value and intimacy, and was perceived to be most beneficial to the relationship. The solid-pseudopapillary tumor (SPT) of the pancreas is a rare type of exocrine pancreatic neoplasm. SPT predominantly affects young women and female children, and is usually discovered incidentally. This tumor is generally benign with a low incidence of malignancy. A 13-year-old girl was admitted to the hospital with a few weeks' history of mild abdominal pain and jaundice. On physical examination, there was no palpable mass. The laboratory tests showed increased SR, CRP, high bilirubin, amylase and lipase serum levels. Ultrasound imaging revealed a solid lesion in the region of the pancreatic head. On MRI, precise tumor localization in the head of the pancreas with pancreatic duct dilatation and compression of the common bile duct were visualized. Pancreaticoduodenectomy and cholecystectomy was performed with good clinical outcome. Microscopic and immunohistochemical studies indicated that tumor cells were typical of SPT without any signs of malignancy. After ...

Lupinus albus L. plants were grown from seeds and irradiated at various stages of development with acute or chronic gamma rays. All plants were greenhouse grown (pre- and post-irradiation) and allowed to proceed through their normal growth cycle. The purpose of these experiments was to establish a Plactochron Index for Lupinus albus L. and to determine the effects of acute and chronic irradiation on development at the macro and microscopic levels. A Plastochron Index was calculated and used as an indirect time scale to evaluate the effects of gamma rays from a common base line. Acute radiation treatment lasted for a period of a few days, whereas chronic treatment was initiated at the seedling stage and lasted for the entire growth season. Vegetative plants were used to study the effects of acute radiation exposure on apical meristem morphology, Plastochron Index, phyllatoxis and gross morphology.

Ignoring the numerous, known and mainly unknown, interactions between chemicals and ionizing radiations, causes strong controversy about the risks of ionizing radiations. Yet the best known interactions between chemicals and radiations is that between radon and tobacco smoke. Although it is and it will be always impossible to derive quite quantitative laws for this interaction, it has been possible to derive some semi-quantitative relationships based on reasonable assumptions and statistics on lung cancer deaths in developed countries. This paper presents a reverse time migration (RTM) method for the migration of shot records in tilted transversely isotropic (TTI) media. It is based on the tilted TI acoustic wave equation that was derived from the dispersion relation. The RTM is a full depth migration allowing for velocity to vary laterally as well as vertically and has no dip limitations. The wave equation is solved by a tenth-order finite difference scheme. Using 2D numerical models, we demonstrate that ignoring the tilt angle will introduce both lateral and vertical shifts in imaging. The shifts can be larger than 0.5 wavelength in the vertical direction and 1.5 wavelength in the lateral direction.

Big bang nucleosynthesis constraints on baryon isocurvature perturbations are determined. A simple model ignoring the effects of the scale of the perturbations is first reviewed. This model is then extended to address the claim that large amplitude perturbations will collapse, preventing their baryons from contributing to the observed baryon density. It is found that baryon isocurvature perturbations are constrained to provide only a slight increase in the density of baryons in the universe over the standard homogeneous model. In particular, it is found that models which rely on power laws and the random phase approximation for the power spectrum are incompatible with big bang nucleosynthesis unless an ad hoc, small-scale cutoff is included. {copyright} {ital 1995 The American Astronomical Society.}

The Belinskii, Khalatnikov and Lifshitz conjecture \\cite{bkl1} posits that on approach to a space-like singularity in general relativity the dynamics are well approximated by `ignoring spatial derivatives in favor of time derivatives.' In \\cite{ahs1} we examined this idea from within a Hamiltonian framework and provided a new formulation of the conjecture in terms of variables well suited to loop quantum gravity. We now present the details of the analytical part of that investigation. While our motivation came from quantum considerations, thanks to some of its new features, our formulation should be useful also for future analytical and numerical investigations within general relativity.

As a result of evidence of the ecotoxicity of nitrofurans, the acute toxicity of furazolidone was tested in vivo on two aquatic organisms, Artemia salina and Daphnia magna, which are both crustaceans. Toxicity studies were also performed on larvae of Culex pipiens molestus. Results indicated a significant toxicity of the compound on Culex pipiens and Daphnia magna, while Artemia salina proved to be the least sensitive.

This review briefly discusses the clinical and basic science rationale for vagus nerve stimulation (VNS) in treatment-resistant depression (TRD). As the number of treatment failures for depression increases, the likelihood of achieving remission during acute treatment decreases, and the risk of relapse increases with the number of treatment failures. Two open trials of adjunctive VNS for TRD showed positive acute results and a growing benefit over time. The results of the acute randomized controlled trial were not significant for the primary outcome (response by HRSD-24), but the secondary measure (IDS-SR-30) was significant for VNS. The operation of a nuclear power reactor necessarily implies the consumption or burnup of reactor fuel by fission and capture, which gives rise to a decrease in the reactivity of the reactor. The effect of americium formation on the criticality of a thermal power reactor using two types of fuel is studied. The three-dimensional core calculation is used to calculate the production of the transuranium isotopes and their effect on the effective multiplication factor (K_e_f_f). This effect cannot be neglected for thermal power reactors with UO_2-PuO_2 fuel (3.11% after 70 weeks of operation). The effect of the transuranium isotopes on the K_e_f_f for a thermal power reactor with UO_2 fuel is about 0.0018% and can be ignored. (author).

Theory of quantum games is relatively new to the literature and its applications to various areas of research are being explored. It is a novel interpretation of strategies and decisions in quantum domain. In the earlier work on quantum games considerable attention was given to the resolution of dilemmas present in corresponding classical games. Two separate quantum schemes were presented by Eisert et al. and Marinatto and Weber to resolve dilemmas in Prisoners' Dilemma and Battle of Sexes games respectively. However for the latter scheme it was argued that dilemma was not resolved. We have modified the quantization scheme of Marinatto and Weber to resolve the dilemma. Simulations of the x-ray free-electron laser (FEL) oscillator are presented that include transverse effects and realistic Bragg crystal properties with the two-dimensional code GINGER. In the present cases considered the radiation divergence is much narrower than the crystal acceptance, and the numerical algorithm can be simplified by ignoring the finite angular bandwidth of the crystal. In this regime GINGER shows that the saturated x-ray pulses have 109 photons and are nearly Fourier-limited with peak powers in excess of 1 MW. Wealso include preliminary results for a four-mirror cavity that can be tuned in wavelength over a few percent, with future plans to incorporate the full transverse response of the Bragg crystals into GINGER to more accurately model this tunable source.

According to the fluid flow characteristics on shellside and ignoring the impact of inlet, outlet and shell wall, a periodic flow unit duct was taken as the simplified model of the shellside to perform numerical simulation by using CFD code, FLUENT. It was found that the both errors in magnitude of the main fluid velocities and heat transfer coefficients between results of simulation and that of experiment or correlations are less than 10%, and the errors of pressure drop between simulation and correlation are no more than 20%. The detailed characteristics and relation of fluid flow and heat transfer on shellside of the RODbaffle heat exchanger were analyzed using the simulation results. (author)

As a result non-formal educational programs for adolescents and adults are given very little support, and this group is virtually ignored in international agreements such as the millennium declaration. Problems can sometimes occur during the construction of cast in-situ concrete piles. These foundations are formed by drilling a substantial hole into the ground and subsequently filling it with steel reinforcement and wet concrete. Several well-documented cases exist where concrete has not filled the hole and substantial voids have been left in the apparently completed pile. A theory has been developed by which it is possible to identify the nature, size and position of a finite defect within a suspect pile. The technique is based on the concept of receptances, and requires information obtained from vibration tests performed on piles after the concrete has hardened, and the nominal dimensions and material properties of the pile. The application of the theory is demonstrated using data relating to a pile with a known defect. The theory described in this paper ignores the effect of the surrounding soil; this can be included by further development of the theory.

The new Code Case to substitute the allowable crack angle for a circumferential surface crack in 'Rules on Fitness-for-Service (FFS)' published from The Japan Society of Mechanical Engineers (JSME). Until now, the angle of a circumferential surface crack was limited below 60deg. This is the limitation with consideration to the stability of the crack if the deep crack penetrates wall of the pipe. Therefore, a long crack (such like an SCC) was obliged to repair or replace even if it was shallow enough. Weld Overlay (WOL) repair in which strength of the original piping is ignored is also inapplicable for the same reason. An acute fish toxicity model was constructed on the basis of a wide series of experimental data for guppy. The Multiple Computer-Automated structure Evaluation program was used to construct the model. The created model possesses very good predictive ability. It can correctly predict acute toxicity for guppy for 80% of compounds with an average error of only 0.63 log unit per median lethal concentration. The importance of the narcosis effect was demonstrated. The main toxicophores, corresponding to polar narcosis and to the reactive chemicals, were identified.

Carbonyl sulfide (COS), a colorless gas, is a side product of industrial procedures sure as coal hydrogenation and gasification. It is structurally related to and is a metabolite of carbon disulfide. COS is metabolized in the body by carbonic anhydrase to hydrogen sulfide (H{sub 2}S), which is thought to be responsible for COS toxicity. No threshold limit value for COS has been established. Results of these studies indicate COS (with an LC{sub 50} of 590 ppm) is slightly less acutely toxic than H{sub 2}S (LC{sub 50} of 440 ppm).

This paper reports the characteristics and local management of 826 acute wounds identified during an audit across all health care providers serving the population of Bradford, UK. To assess the potential of CT in evaluating the histological severity of acute appendicitis in comparison with surgical and pathological findings. The CT images of 75 patients with surgically proven appendicitis, including 10 cases of catarrhal, 34 of phlegmonous, and 31 of gangrenous appendicitis, were retrospectively analyzed for the following five CT findings: hazy periappendiceal densities, enlarged appendix, increased enhancement of the appendiceal wall, increased enhancement of the periappendiceal intestinal wall, and deficiency of the appendiceal wall. By comparing all the CT findings and the pathological severity of appendicitis (catarrhal, phlegmonous, and gangrenous), the prevalence of the five CT findings was calculated for each pathological category. Pharmacokinetics is studied, radiation doses to patients when using domestic thallium-201 chloride in diagnosis of the heart ischemic disease is estimated; contribution into the total dose of thallium-201 (1.5 %), thallium-201 (0.2 %) and lead-201 + lead-203 (0.05 %) radionuclide impurities is also determined. Internal doses were estimated for a heterogeneous phantom of a standard man; the calculations were carried out using a computer. Scanning of patients which were treated with radiopharmaceuticals to be diagnosed was carried out using a whole body counter and gamma-chamber. Large intestine, kidneys and thyroid are the critical organs when using thallium-201 chloride. Limitations of traveling wave relaying schemes for protection of overhead extra-high voltage transmission lines are investigated. A method of analysis of traveling wave phenomena for three phase transmission lines is developed in which the interdependent phase voltages and currents are decoupled into their modal counterparts, which are approximately independent. A time domain digital simulation program is used to solve the modal transmission line equations to obtain the fault induced traveling waves detected at the relay location. The frequency dependence of the aerial modes is ignored but their losses are included. A lumped element analysis method, originally developed for transient analysis of lossy coaxial cables, is adapted here to obtain approximate solution for the fault induced traveling waves of the ground mode. This is due mainly to lack of legislation, information and modern treatment and disposal facilities designed for this purpose. A nation-wide questionnaire survey was conducted to asses the status of hospital waste management. The study started from October 1997 till August 1998. We found that 75% of the surveyed hospitals completely ignore their total waste quantity: 73% of hospitals surveyed practice segregation at source of infectious, pathological, sharps and pharmaceuticals; more than 40% dispose of their hospital risk wastes through the municipality waste disposal, 24% by burning in open fires, 14% by on-site hospital incinerators, 11% in on-site dumping, 8% handled by a private contractor and 1% in uncontrolled landfill. The present work concerns the numeric modeling of the sea-wave instability under the effect of the logarithmic wind at hurricane conditions (ignoring non-linear effects, such as wave breaking, foam production, etc. Powell et al. (2003)^1, Shtemler et al. (2003)^2. The central point of the study is the calculation of the growth rate, which is proportional to the fractional input energy from the wind to the wave exponentially varied with time. The present modeling demonstrates that the Miles-type model applying Charnock's formula Charnock (1955)^3 for roughness to the hurricane -wind parameters underestimates the growth rate of the wind waves 5-40 times as compared with the model employing the roughness and friction velocity adopted from experimental data for hurricane winds.^1 This occurs due to Charnock's formula fails at large wind speeds. The standard approach is to assume that, as temperatures heat up during star formation, methanol and other species evaporate from grain surfaces and undergo a warm gas-phase chemistry at 100 K or greater to produce species such as methyl formate, dimethyl ether, and others. But a series of laboratory results shows that protonated ions, typical precursors to final products in ion-molecule schemes, tend to fragment upon dissociative recombination with electrons rather than just ejecting a hydrogen atom. The cell is modeled as an ellipsoid with shell and the surface stress distribution calculated by means of the Maxwell Stress Tensor. The cell membrane is treated as incompressible material having both bending and shear energies of deformation. Employing the principle of virtual work, cell volume (and hence shape) is computed as a function of the external field strength and orientation with respect to the field. The following results were obtained: (1) The surface stresses that develop or the cell membrane are distributed having both normal and tangential components that act together to produce a rounding of the cell. A method for determining the reactivity of subcritical systems of fissile material, using neutron-noise power spectral density measurements in conjunction with a /sup 252/Cf source, has been tested in experiments with an aqueous solution containing uranium fluoride (4.95 wt% /sup 235/U). The kappasub(eff)-values obtained from the ratio of spectral densities G*/sub 12/G/sub 13//G/sub 11/G/sub 23/ agreed with those from break-frequency noise analysis and with bias-corrected transport theory calculations within the statistical uncertainty of the measurements. Corrections for the effects of spatial modes and source-detector locations appear to be well-understood, since measurements for a variety of source-detector positions resulted in essentially the same kappasub(eff)-values. Non-axisymmetric forces are presented for 107 spiral galaxies using the 2 Micron All Sky Survey (2MASS) images. We apply both Cartesian integration and a polar grid integration utilizing a limited number of azimuthal Fourier components of density. We found that bar strength is independent of the method used to evaluate the gravitational potential. However, the polar method is more suitable for weak and noisy images. Bar strength was found to be sensitive to the Hubble-type dependent scale height of the disk, which has been ignored in the previous studies. On the other hand, the method is rather insensitive to the vertical model of the disk, as long as a same vertical dispersion is assumed, or to the boxy/peanut shaped structure, studied in terms of non-constant vertical scale height along the disk. Although local thrombolysis may achieve recanalization and improve outcome, mortality is still between 35% and 75%. Early treatment is a fundamental factor, but detailed studies of the exact time management of the diagnostic and interventional workflow are still lacking. The authors hypothesized that acute respiratory responsiveness to ozone predicts chronic lung injury from repeated exposure to ozone-containing air pollution. They tested this hypothesis in 164 middle-aged nonsmoking residents of an ozone-polluted community who underwent lung-function measurements during 1986 and 1987 (i.e., time 3). The time-3 study was a follow up of more comprehensive studies conducted in 1977--1978 (time 1) and in 1982--1983 (time 2). We report a new case of Lyme disease with cardiac manifestations, which has been possible to follow during the long period of 12 years. We have detected the usual ECG abnormalities, and concentric hypertrophic myocardiopathy, by echocardiography. The acute myocarditis was demonstrated by /sup 111/In-antimyosin scintigraphy, which showed global myocardial uptake of the tracer, constituting the first report, to our knowledge, of Lyme myocarditis diagnosed by this method.

Context: The Balanced Budget Act (BBA) of 1997 allowed some rural hospitals meeting certain requirements to convert to Critical Access Hospitals (CAHs) and changed their Medicare reimbursement from prospective to cost-based. Some subsequent CAH-related laws reduced restrictions and increased payments, and the number of CAHs grew rapidly. Purpose: To examine factors related to hospitals' decisions to convert and time to CAH conversion. Methods: Eighty-nine rural hospitals in Iowa were characterized and observed from 1998 to 2005. Cox proportional hazards models were used to identify the determinants of time to CAH conversion. The value of biliary scintigraphy was studied in 180 patients with suspected biliary tract disease. Most of the patients were investigated additionally by conventional techniques such as cholecystography, cholangiography and ultrasonography. It is concluded that biliary scintigraphy is a simple and safe technique for visualization of the biliary tract. It is particularly useful in the evaluation of acute cholecystitis, in patients with iodine sensitivity obstructive from nonobstructive jaundice.

The adipocytokine adiponectin is released by adipocytes upon activation of the peroxisome proliferator-activated receptor gamma (PPAR gamma). PPAR gamma has binding sites for thiazolidinediones and free fatty acids (FFAs). To evaluate if adiponectin serum concentrations are synergistically regulated by FFAs and thiazolidinediones IN VIVO plasma FFAs were acutely elevated in healthy subjects pre-treated with rosiglitazone or placebo. Sixteen healthy male subjects (23-37 years) were included in this double-blind, randomized, placebo-controlled parallel-group study. Rosiglitazone 8 mg or placebo was administered daily for 21 days. We reported three cases of acute intracranial tuberculosis including miliary tuberculosis, basal meningitis, tuberculomas and neuritis of cranial nerves. All patients had native and contrast enhanced CT and MRI scans. MRI revealed more granulomas and a better imaging contrast in the detection of basal meningitis. Serum ferritin levels were measured in 72 normal subjects and in 214 cases with various diseases by an immunoradiometric assay. In normal subjects, the serum ferritin levels were 27-230 ng/ml. Elevated serum ferritins were observed in most cases with iron excess and acute hepatitis. Markedly elevated levels were found in the majority of cases with acute leukemia, malignant lymphoma, hepatoma, and pancreatic cancer. High ferritin levels were also found in other malignant diseases. However, the range overlapped broadly with that of nonmalignant diseases. The serum ferritin correlated significantly with serum iron in normals and in those with iron deficiency anemia. In most nonmalignant cases, the serum ferritin and iron levels distributed on a regression line obtained from levels in normals and those with iron deficiency anemia. In August 1982, a 44-year-old man was diagnosed as having rectal cancer, histologically diagnosed as well differentiated adenocarcinoma, and abdominoperineal resection and colostomy were performed. Postoperatively, he received chemotherapy with mitomycin C up to a total dose of 100 mg. In September 1986, lung metastasis occurred and he was treated with a combination chemotherapy consisting of cisplatin, pirarubicin and 5-fluorouracil. In the following year, radiation treatment (total: 6900 rad) was given for a recurrent pelvic lesion. Peripheral blood on April 30, 1988, showed anemia, thrombocytopenia and appearance of myeloblasts, and a diagnosis of acute myelogenous leukemia (FAB: M1) was made. A total of 272 tap water samples were collected from 68 homes throughout the city of Tehran. Analysis for cadmium, zinc, lead, copper, arsenic, iron and manganese showed some accumulation of these metals in household piping overnight. However, the concentration of all metals was in the parts per billion (ug/l) range and well below international standards. In acute intoxication, uranium (U) not only inhibits bone formation but its excretion in urine also causes renal damage. The former effect is ameliorated by tetracycline (TC), probably due to its chelation property, which might also prevent U deposition in bone. Chemical determination of U incorporated in bone and a histological study of the kidneys were performed on animals injected with U and then treated with TC. The results showed that TC was unable to prevent the binding of U to bone while it exacerbated U-induced renal damage.

Cross-sectional imaging has revolutionised the radiological diagnosis of pleural collections. Not only can the precise location and volume of a pleural effusion be established, but also features specific for the aetiology of the effusion can be demonstrated. Increasingly, radiologists are called upon to perform image-guided biopsies, aspirations and small bore chest drain placement, all of which have been shown to be safe and efficacious. Pneumothoraces occurring due to acute trauma and in an intensive care setting can also benefit from radiological input, both in terms of diagnosis and image-guided treatment. (orig.)

We report the cranial CT and MRI findings in three children with Lyme disease (neuroborreliosis). The neuroimaging findings in children have been rarely reported. We found cranial MRI far superior to cranial CT. Ring-enhancing lesions have been described in acute disseminating encephalomyelitis and multiple sclerosis but not in neuroborreliosis. Although other infectious and inflammatory diseases cannot be excluded, Lyme disease should be included in the differential diagnosis and put forward as being the most likely diagnosis in the appropriate clinical setting. Anesthetic techniques vary widely in the endovascular repair of abdominal aortic aneurysms (EVAR). Previous studies have demonstrated the feasibility of using local anesthesia. However, the ideal anesthetic technique has not been determined. This study examines whether anesthesic technique influences the outcomes of EVAR. Data regarding demographics, risk factors, procedural characteristics, recovery characteristics, treatment complications, acute ( 0.05, ANOVA). From these results we concluded that EVAR with local anesthesia is a safe and efficacious method that may reduce recovery times and postoperative medical morbidity compared to use of genera1 or spinal/epidural anesthesia.

A 3"1/_2-year-old child developed unilateral diaphragmatic paralysis after chest drain insertion. Plain chest X-ray demonstrated paravertebral positioning of the chest-tube tip, and magnetic resonance imaging revealed hematomas in the region of the chest-tube tip and the phrenic nerve fibers. The trauma to the phrenic nerve was apparently secondary to malposition of the chest tube. This is a rare complication and has been reported mainly in neonates. Radiologists should notify the treating physicians that the correct position of a chest drain tip is at least 2 cm distant from the vertebrae. (orig.)

This study was designed to identify abnormalities in the hepatic-arterial/portal-venous scintiangiogram (SA) in alcoholic hepatitis (AH). SA's were performed in 35 patients with acute alcoholic hepatitis (AAH), 8; acute alcoholic hepatitis superimposed on cirrhosis (A/C), 14; and cirrhosis (C), 13. Posterior flows were done with a bolus of 10 mCi Tc-99m sulfur colloid with computer time-activity curves over the liver and left kidney. Curves were analyzed for per cent of hepatic arterial (HA) and portal venous contribution using the slope ratio method. Hepatic arterialization was estimated from the angle of the HA component of the curve. Reversal of the relative contribution of the hepatic and portal components of total flow were seen in all groups. 1. Choline, and the choline analogues monoethylcholine (MEC) and N-aminodeanol (NAD) were examined for prophylactic activity in acute acetylcholinesterase inhibitor toxicity in mice. The rank order of potency of the compounds was MEC greater than NAD greater than choline. 2. Simultaneous administration of MEC (60 mg kg-1) or NAD (200 mg kg-1) with physostigmine reduced lethality to 17 and 13% respectively. MEC (60 mg kg-1) completely protected against disopropylfluorophosphate (DFP) and diazinon toxicity, and NAD reduced lethality to 17% for both agents. Objective. To compare turbo inversion recovery magnitude (TIRM) with standard T1-weighted (T1-W) and T2-weighted (T2-W) MR sequences in the very early detection of acute osteomyelitis in children. Materials and methods. In 15 children with osteomyelitis, 15 sets of T1-W spin-echo (SE) (TR/TE, 400-640/12-17), T2-W turbo spin-echo (TSE) (TR/TE/ETL, 3290-4465/112-120/11), and TIRM (TR/TE/TI, 4000-6120/60/160) images were acquired with a 1.0-T magnet. Contrast-to-noise (C/N) ratios and percentage of signal between lesion and normal bone marrow were analysed with a computer-assisted image analysing system in a region of interest (ROI). Results. Average-risk subjects of two quintiles (50-54 and 65-69 years) were randomly selected from the electoral roll and invited to participate in a VC-based CRC screening program. After bowel preparation, bowel insufflation with CO_2 and intravenous hyoscine butylbromide (unless contra-indicated), supine and prone scans were acquired with a single array GE CTi helical scanner. When exposure-calibrated ionization chambers are used to calibrate megavoltage X-ray and electron beam therapy machines, the calibration factor can be divided into a chamber-dependent and a chamber-independent component. Until recently, only the chamber-independent part, which has been derived theoretically, has been used; the chamber-dependent component has been ignored. Recent experimental and theoretical considerations, however, have indicated that the dependent component must be taken into account and can affect the calibration by as much as 4%. The calibration can change due to (i) the thickness of the chamber wall, (ii) the chamber wall composition, i.e. whether the chamber is constructed of air-equivalent material, or tissue- or water-equivalent material, and (iii) the size of the chamber changing the displacement factor. A feature of the Fast Flux Test Facility (FFTF) is that fuel assemblies can be stored within the reactor vessel, either for future use in the core or for temporary storage of spent fuel. Due to the close proximity of fuel assemblies stored in the in-vessel storage (IVS) locations to the ex-vessel flux monitors (EVFMs), neutrons originating in the stored fuel can result in significant contributions to the total EVFM signals. The paper shows EVFM-induced electronic current data during operation at full power for FFTF power cycles 1 through 7. The data were obtained early in each cycle so that signal drift due to core burnup could be ignored. The differences in currents are attributed to differences in cycle-to-cycle core and stored fuel loadings. At the beginning of cycle 1 there were no fuel assemblies stored in IVS. An extensive study of the effects of correlations on both charged and neutral current weak interaction rates in dense matter is performed. Both strong and electromagnetic correlations are considered. The propagation of particle-hole interactions in the medium plays an important role in determining the neutrino mean free paths. The effects due to Pauli blocking and density, spin, and isospin correlations in the medium significantly reduce the neutrino cross sections. As a result of the lack of experimental information at high density, these correlations are necessarily model dependent. For example, spin correlations in nonrelativistic models are found to lead to larger suppressions of neutrino cross sections compared to those of relativistic models. This is due to the tendency of the nonrelativistic models to develop spin instabilities. In the treatment of prostatic cancer using a newly proposed intermittent multiportal therapy (DIMT), acute reactions, change in tumor marker levels, and complications more than 6 months after treatment were evaluated. From June 1989 to September 1990, 26 patients with prostatic cancer (stage A2, 3 patients; B, 9; C, 2; D, 10; recurrence, 2) were treated. Fifteen patients were followed up for more than one year, with a mean period of 11.6 months. The treatment schedule is 52.5 Gy in 16 fractions in 4 weeks for radical treatment, and 30.0 Gy in 8 fractions in 2 weeks for palliative treatment. The 360 degree rotation about the patient was divided into 16 fractions, and 2 opposing fractions were used in one day as parallel opposed portals to treat the target volume. The authors made peritoneal dialisis with 5% of glucose (7 ml/kg of body weight) in 11 dogs under the conditions of an acute experiments. They examined cortiosl and ADH activity, hematocrite and plasma protein in the samples of blood, obtained on the 20th and 50th minute. ADH was titrated biologicaly by a new method, proposed by Nacev. The results were compared with the changes in the circulatory and renal indices, obtained at the same procedure in the preceding investigations. There was an increase in the cortisol and ADH activity, which could be explained by the total hypovolemia, induced by peritoneal dialisis. The increase of the cortisol level is described as a separate link in a more complex mechanism, assuring metabolic homeostasis. PMID:1222705

Two patients who developed large intracranial vessel occlusion after standard radiation therapy for brain tumor are described. This form of vascular occlusion is usually seen in patients who have previously been treated by radiotherapy for intracranial tumor who then develop a relatively acute change in neurologic status. Histology of the lesion mimics accelerated focal arteriosclerosis. The clinical and radiographic manifestations of one case were highly atypical. The vasculopathy became evident shortly after termination of radiation therapy for a fourth ventricular ependymoma, and the angiographic picture stimulated a diffuse arteritis. The second patient was more typical, with clinical symptoms developing 12 years after radiation therapy for an oligodendroglioma. Local control of Ewing's sarcoma was analysed in a series of 67 patients treated by surgery and/or radiotherapy as well as combination chemotherapy. Radiotherapy was employed with or without surgery in 60 patients and produced an overall local control rate of 55%; complete excision of the primary lesion seemed to be beneficial. There was a marked variation in control rates depending on the site of the primary lesion: limb 85%, rib 53%, pelvis 31% and other sites 33%.Primary tumours greater than 10 cm in diameter were significantly less likely to be controlled. Using daily fractions of approximately 180 cGy, total doses in excess of 6000 cGy seem more likely to produce serious late morbidity amd may not increase the local control rate. No cases of second malignancy arising in irradiated tissue have been observed to date, but one patient developed acute lymphoblastic leukaemia. (author).

The initial series of laparoscopic single stage orchiopexy for the abdominal or emergent impalpable undescended testis is reported. The first laparoscopic orchiopexy was performed at our institution in October 1991. Between October 1991 and January 1993, 14 patients (16 testes) underwent minimally invasive surgery with 6 months of followup in all cases. No evidence of testicular loss or acute atrophy has occurred, with the entire procedure being accomplished by laparo-endoscopic techniques in all cases. Treatment of 3 of the 16 testicles consisted of stage 2 of staged orchiopexy, and to our knowledge these cases represent the first stage 2 orchiopexy completed by laparoscopic techniques. In those cases stage 1 was performed by laparoscopic clipping of the vessels. The details of the procedure as we now perform it are described. PMID:7915336

Iodo(/sup 14/C)antipyrine autoradiography was used to measure gastric mucosal blood flow in anesthetized rats and to study regional distribution. Blood flows of 61 +/- 8 ml.100 g-1.min-1 (means +/- SE) in corpus and 84 +/- 9 ml.100 g-1.min-1 in antral mucosa compared well with previously reported measurements by hydrogen clearance. Blood flow in the crests of corpus mucosal folds was significantly higher than in the valleys between folds, indicating that the greater susceptibility of the former areas to acute injury, documented in several studies, is not associated with a perfusion defect in the resting stomach. Corpus mucosal blood flow was also higher in the side walls of the stomach than in the greater curvature region, and in distal than in proximal locations. No systematic regional variations within antral mucosa were demonstrated.
