Purpose: To determine the rankings of 6 input-output functions for understanding low-level, conversational, and high-level speech in multitalker babble without manipulating volume control for listeners with normal hearing, flat sensorineural hearing loss, and mildly sloping sensorineural hearing loss. Method: Peak clipping, compression limiting, and 4 wide dynamic range compression (WDRC) input-output functions were compared in a repeated-measure design. Interactions among the compression characteristics were minimized. Speech and babble were processed and recorded at 3 input levels: 45, 65, and 90 dB sound pressure level. Nonlinear distortion added by the loudspeaker in a hearing aid lowers the signal-to-noise ratio and may degrade the hearing aid user's ability to understand speech. The balancedarmature- type loudspeakers, predominantly used in hearing aids, are inherently nonlinear devices, as any displacement of the loudspeaker diaphragm inevitably changes the magnetic and electrical characteristics of the loudspeaker. A numerical time-domain model capable of describing these nonlinearities is presented. By simulation it is demonstrated how the output distortion could potentially be reduced significantly through careful design of the mechanical properties of the armature.

Idiopathic renal hematuria in a military working dog.

The diagnosis of idiopathic pulmonary hemosiderosis (IPH) may be elusive. A 6-year-old boy had microcytic hypochromic anemia and a hemolytic component. Hemosiderin-laden macrophages were not found in the gastric aspirate. He had no pulmonary signs or symptoms. Extensive hematologic and roentgenologic investigations failed to reveal the cause of the anemia. Quantitative serial scintigraphic scanning showed significant (35%) pulmonary sequestration of autologous erythrocytes labeled with sodium chromate Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy spece Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy specimen confirmed the diagnosis of IPH. A diagnosis of IPH should be considered when children have iron deficiency anemia and pulmonary signs or symptoms. Organ sequestration studies may be helpful in equivocal cases.

A 1.5-year-old male Belgian Malinosis Military Working Dog presented with a 1-month history of intermittent hematuria. Diagnostic ultrasound and contrast radiography demonstrated large blood clots in the urinary bladder and a filling defect in the right renal pelvis. At surgery, clotted blood was present in the right ureter and bladder. Following right nephrectomy, the dog returned to training. One month later, elevations in urea nitrogen and creatinine were noted. Hematuria recurred at 3 months and the dog was found dead in its kennel. Treatment of asymptomatic internal resorption of a maxillary premolar tooth in a military working dog.

Hereditary hemorrhagic telangiectasia (Osler-Rendu-Weber disease) is a genetic disorder with autosomal dominance, variable penetrance, and an estimated prevalence of 1/10,000 inhabitants in France. Diagnosis is based on clinical criteria including epistaxis, telangiectasia, visceral manifestations, and familial occurrence. Pulmonary arteriovenous malformations, present in 15-33% of patients, are its primary visceral complications. The disease may be revealed by infectious and ischemic neurological manifestations due to paradoxical embolism. The high frequency of neurologic complications even in asymptomatic patients justifies systematic screening for pulmonary arteriovenous malformations. Treatment of these malformations is based on percutaneous transcatheter coil embolization of the feeding artery. Pulmonary arterial hypertension is rare in this disease. The use of peritoneal venous shunting for intractable neonatal ascites: a short case series

An asymptomatic pink discoloration of a maxillary right fourth premolar tooth was discovered during a routine oral examination on a 9 year-old Belgian Malinois dog. A radiolucent lesion was seen in the pulpal chamber on radiographic examination. The lesion had perforated the mesiobuccal root of the tooth. The primary differential diagnosis was idiopathic internal resorption. The tooth was treated by partial resection (removal of the mesiobuccal root and associated crown). A vital pulpotomy and amalgam restoration was performed on the remaining tooth structure. A follow-up 1 year later demonstrated a successful treatment outcome. The animal was asymptomatic and able to perform military duties. Clinical and radiographic signs of healing were evident and the tooth was functional. PMID:10518874

Although they are very distinct and well-defined entities, there is clinical and genetic evidence supporting some degree of overlap between the pathogenesis of the two. Subclinical gut inflammation is present in up to two thirds of all SpA patients and can evolve into IBD. A voice-tracking algorithm was developed and tested for the purposes of electronically separating the voice signals of simultaneous talkers. Many individuals suffer from hearing disorders that often inhibit their ability to focus on a single speaker in a multiple speaker environment (the cocktail party effect). Digital hearing aid technology makes it possible to implement complex algorithms for speech processing in both the time and frequency domains. In this work, an average magnitude difference function (AMDF) was performed on mixed voice signals in order to determine the fundamental frequencies present in the signals. A time prediction neural network was trained to recognize normal human voice inflection patterns, including rising, falling, rising-falling, and falling-rising patterns. The movement from traditional regulatory hearings to negotiated settlements represents both a departure from cost of service regulation and a relaxation of regulatory oversight. Under negotiation parties are able to renegotiate inclusions in their cost of service while simultaneously creating a profit margin for the regulated firm where none existed under the cost of service outcome of a traditional hearing. This paper constructs a model to illustrate the existence of positive gains to pipeline and shipper from the re-allocation of expenses through time in the regulated pipeline services market in Canada. Recent process changes and developments in the areas of solvent extraction and UO_3 production together with new processes for the production of ceramic UO_2 and UF_4, are described. Environmental and waste management problems, progress and solutions are next considered. One-year follow-up of the effects of sildenafil on pulmonary arterial hypertension and veno-occlusive disease

Individuals with male factor infertility resulting from idiopathic oligo-, astheno- or teratozoospermia are frequently offered IVF in an attempt to increase their chances of having a child. A concern remains whether these infertile males have an elevated risk of transmitting chromosomal abnormalities to their offspring. Sperm chromosomal complements from these men were assayed using the human sperm/hamster oocyte fusion system and fluorescence in situ hybridization (FISH) on sperm nuclei. For each of 5 infertile patients, 100 sperm karyotypes were analyzed and multicolour FISH analysis was performed on a minimum of 10,000 sperm nuclei for each chromosome-specific DNA probe for chromosomes 1 (pUC1.77), 12 (D12Z3), X (XC) and Y (DYZ3). Abstract in english We hypothesized that chronic oral administration of the phosphodiesterase-5 inhibitor sildenafil could improve the exercise capacity and pulmonary hemodynamics in patients with pulmonary arterial hypertension (PAH) on the basis of previous short-term studies. We tested this hypothesis in 14 subjects with PAH, including seven patients with the idiopathic form and seven patients with atrial septal defects, but no other congenital heart abnormalities. Patients were subjected (more) to a 6-min walk test and dyspnea was graded according to the Borg scale. Pulmonary flow and pressures were measured by Doppler echocardiography. Patients were given sildenafil, 75 mg orally three times a day, and followed up for 1 year. Citrate is an important naturally occurring inhibitor of calcium stone formation in urine. Urinary citrate excretion was examined in 43 consecutive patients with recurrent idiopathic calcium nephrolithiasis and in 50 normal controls by a specific enzymatic technique. Hypocitraturia (<1.6 mmol/24h) was found in 14 (33%) stone formers compared to 6 (12%) normal controls (p = 0.03). Citrate excretion expressed as citrate-creatinine ratio in 24 hour urine samples was significantly lower in the stone formers than in the healthy controls (p = 0.03), and significantly lower in healthy men, compared to healthy females (p = 0.006). There was a great variability in urinary citrate levels in both groups, and a considerable overlap in the urinary citrate excretion between normal subjects and stone formers. The composition upon a relatively short period of cure at low temperature forming a solid material which fills the cracks and fissures; curing the filler composition; and, applying a sealant composition to the surface of the heat exchanger at the filled leakage site. Dr. Rudolf Schulten of West Germany and expert witnesses from national laboratories, utilities, and the nuclear industry testified on reactor safety issues as they relate to the Chernobyl accident and public concern that modern technology has not paid enough attention to public safety. Each of the witnesses contributed safety-related information based on what has been learned from the Soviet incident. Particular focus went to similarities and differences between the Chernobyl and US reactors in safety design and engineering and to the environmental effects of the accident. Details of a 1 day conference designed to create more efficient interactions between the gas supply and electricity systems in Ontario and new models for gas supply and the electricity system were presented. Market efficiency, generators' perspectives on critical gas issues, an overview of the North American gas and an outline of issues concerning Ontario were discussed. A natural gas industry forecast and issues concerning marketers and producers were presented. The terms on which gas is bought and sold were discussed. Regulatory tools, cost allocation and issues relating to unbundling were also presented. Infrastructure needs were reviewed, as well as basic market structures. It was suggested that appropriate commercial service options need to be developed and approved, including cross-utility services.

Computed tomography of middle ear cholesteatomas without tympanic membrane perforation

Coilguns have the ability to provide magnetic pressure to projectiles which results in near constant acceleration. However, to achieve this performance and control projectile hearing, significant constraints are placed on the design of the coils. We are developing coils to produce an effective projectile base pressure of 100 MPa (1kbar) as a step toward reaching base pressures of 200 MPa. The design uses a scalable technology applicable to the entire range of breech to muzzle coils of a multi-stage launcher. This paper presents the design of capacitor-driven coils for launching nominal 50 mm, 350 gram projectiles. Design criteria, constraints, mechanical stress analysis, launcher performance, and test results are discussed.

Block Talk: Spatial Language During Block Play

The growth of a middle ear cholesteatoma behind a normal tympanic membrane is a rate though possible event. In such cases, CT may provide useful information for diagnosis. The results are presented of a CT study carried out on 14 patients affected with unilateral conductive hearing loss and with normal tympanic membrane. CT allowed the diagnosis of meddle ear cholesteatoma to be made in all cases. All patients were treated with surgery: 8 of them underwent tympanoplasty and 6 explorative tympanotomy. While the diagnosis of cholesteatoma was confirmed in 13 patients, in 1 case tympanosclerosis was diagnosed. CT diagnosis of middle ear cholesteatoma is based on the demonstration of a low-density soft-tissue mass, in association with bone erosion or ossicular dislocation. The author emphasizes the difficulty of a CT diagnosis of cholesteatoma in the patients with middle ear soft-tissue masses in the absence of bone alterations.

A 52-year-old woman had a history of left hearing loss for 5 years. An acoustic neurinoma with 3.2 cm in diameter was diagnosed and treated with gamma-knife radiosurgery (19 Gy of marginal dose) 1 year and 4 months ago. She developed headache, nausea, and visual disturbance 1 month prior to admission. Slight left facial palsy appeared after radiosurgery. Magnetic resonance imaging demonstrated the tumor with central necrosis in the left cerebellopontine angle cistern, increasing in size to 3.5 cm in diameter, and hydrocephalus. Tumor removal was performed incompletely, because of the fibrous appearance of the tumor and severe adherence with the surrounding cerebellar tissue. Facial palsy did not worsen after surgery. Since the hydrocephalus was not resolved, a right ventriculo-peritoneal shunt was inserted. The clinical course in this case suggests that tumor removal followed by radiosurgery was an approximately effective treatment for large acoustic neurinoma. ...

From Light to Consciousness Journey to the End of Science or Fin de Siecle Malaise

The chicken extracellular matrix glycoprotein ES/130 is necessary for epithelial-mesenchymal transformation in the developing hear and is also expressed in noncardiac chicken tissues such as limb and notochord. We have identified hES, the human homology of chicken ES/130. Fluorescence in situ hybridization analysis (FISH) localizes hES to human chromosome 20p11.2-p12. FISH analyses of individuals with 20p12 deletions and affected by Alagille syndrome exclude hES as a candidate gene for this disorder. Reverse transcriptase-polymerase chain reaction studies reveal that hES is expressed in both fetal and adult human tissues and that hES expression in the left ventricle is increased in the failing adult heart. Further studies will evaluate how hES mutations may relate to congenital human cardiac and skeletal anomalies as well as cardiac remodeling in the adult. 16 refs., 2 figs.

A study on brain CT was made in 20 cases of typical endemic cretinism and 15 cases of normal persons. These patients from severely iodine deficient area were diagnosed as neurological endemic cretinism characterized by severe mental retardation with impairment of hearing, speech, neuromotor, etc. to varying degrees. The brain CT showed that there were many and deep depressions in cerebral cortex, especially in frontal and parietal lobes of some cretins, the lateral ventricle system was dilated, particularly in posterior part of it, and the interhemisphere fissure, lateral fissure and subarachnoid cisterns were also expanded. The CT value of cortex was higher and white matter was lower than that of normal persons. In some severe cases, the two hemispheres of brain were not the same in size. To assess the utility of magnetic resonance (MR) imaging in the study of pediatric patients with clinical suspicion of pituitary disease. We studied 18 patients aged 7 to 18 years.Fifteen had hormonal disturbances, two presented amenorrhea and 1 complained of headache, fever and symptoms of polyuria and polydipsia. Assessment of bone formation and bone resorption in osteoporosis: a comparison between tetracycline-based iliac histomorphometry and whole body /sup 85/Sr kinetics

Full text: Disorders of brain development and phakomatosis are resulting from disturbed embryonic-foetal development One third of all major embryological anomalies involve CNS, and over 2000 different anomalies have been described. Anomalies of the brain often cause foetal and neonatal death, and mental and physical retardation in pediatric group. The majority of disorders of brain development and phakomatosis are idiopathic, and most of them are not hereditary or familial. Ultrasonography plays the important role in screening foetal and neonatal brain, but after closure of fontanels it is difficult to find the acoustic window. CT has limited contrast resolution, and disadvantage exposing infant to ionizing radiation. It is helpful to demonstrate the presence of calcifications. Video equipment of tele dosimetry and audio

Bone formation and resorption have been measured in patients with idiopathic osteoporosis by histomorphometry of 7.5-mm trephine biopsies and in the whole body by 85Sr radiotracer methodology and calcium balances. The studies were synchronized and most were preceded by double in vivo tetracycline labeling. Correlations between histological and kinetic bone formation indices were better when better when based on the extent of double tetracycline labels than on measurements of osteoid by visible light microscopy. Correction of the kinetic data for long-term exchange, using 5 months' serial whole body counting of retained 85Sr, improved the fit of the kinetic to the histological data. A statistical analysis of the measurement uncertainties showed that the residual scatter in the best correlations (between exchange-corrected bone formation rates and double-labeled osteoid surface indices) could be attributed to measurement imprecision alone. Recent environment, energy and resources cases and issues in Alberta

Since mid-March the Commission on Water Management, with the assistance of the appropriate ministries, visited all regions of the province in order to acquaint themselves first hand with regional water management problems. During the month of June, the Commission also organized a series of ten workshops, attended by a limited number of experts, to explore in depth certain specific questions in the area of water management. During these public sessions the Commission hoped to discuss specifically the specific questions relating to water management problems of First Nations communities, bearing in mind the essential nature of water in maintaining traditional activities. The workshop took place on the Montanard reserve of Mashteuiatsh (Pointe-Bleu). The Mackenzie Valley Pipeline Project was reviewed in relation to the Deh Cho First Nation's advanced actions in the federal court to enjoin the review panel from proceeding with the review process. The Deh Cho First Nation is seeking a declaration that the plan violates their rights under the Canadian Charter of Rights and Freedoms and the Constitution Act. Various regulatory and government agencies have now developed a plan to coordinate their different regulatory processes and approvals in the Cooperation Plan for the Environmental Impact Assessment and Regulatory Review of the Northern Gas Pipeline project. The Deh Cho allege that they were not afforded the opportunity to participate in the development and implementation of the plan. A review panel plans to proceed once the environmental and social impacts are determined, and the consortiums plan to minimize harm has been submitted. ...
