A 1.5-year-old male Belgian Malinosis Military Working Dog presented with a 1-month history of intermittent hematuria. Diagnostic ultrasound and contrast radiography demonstrated large blood clots in the urinary bladder and a filling defect in the right renal pelvis. At surgery, clotted blood was present in the right ureter and bladder. Following right nephrectomy, the dog returned to training. One month later, elevations in urea nitrogen and creatinine were noted. Hematuria recurred at 3 months and the dog was found dead in its kennel. Necropsy showed a blood-filled left renal pelvis and ureter. PMID:1454185

Spondylarthritides (SpA) and inflammatory bowel disease (IBD) are idiopathic, chronic inflammatory disorders. Although they are very distinct and well-defined entities, there is clinical and genetic evidence supporting some degree of overlap between the pathogenesis of the two. Subclinical gut inflammation is present in up to two thirds of all SpA patients and can evolve into IBD. This subclinical gut inflammation has been shown to be strongly associated with joint inflammation, providing a clue for a common pathophysiologic background. The management usually consists of observation, diuretics, paracentesis, albumin replacements, and self correction. However, in some cases, the above treatment remains unsuccessful. Results: We present 2 cases of intractable ascites causing metabolic abnormalities, severe protein and immunoglobulin loss, and respiratory compromise. To assess the utility of magnetic resonance (MR) imaging in the study of pediatric patients with clinical suspicion of pituitary disease. We studied 18 patients aged 7 to 18 years.Fifteen had hormonal disturbances, two presented amenorrhea and 1 complained of headache, fever and symptoms of polyuria and polydipsia. All the patients were examined using a Siemens SP 42 1-Tesla MRI scanner. Sagittal and coronal T1-weighted spin-echo images were obtained; in addition T2-weighted spin-echo or fast spin-echo imaging was performed in ten cases and intravenous gadolinium was administered in nine. We found 9 patients with hypothalamic-pituitary dysgenesis, 2 with germinoma, 2 cases of pituitary hemosiderosis in patients with thalassemia, 2 cases of microadenoma, one abscess, one case of idiopathic central diabetes insipidus and one of Langerhans cell histiocytosis. The diagnosis of idiopathic pulmonary hemosiderosis (IPH) may be elusive. A 6-year-old boy had microcytic hypochromic anemia and a hemolytic component. Hemosiderin-laden macrophages were not found in the gastric aspirate. He had no pulmonary signs or symptoms. Extensive hematologic and roentgenologic investigations failed to reveal the cause of the anemia. Quantitative serial scintigraphic scanning showed significant (35%) pulmonary sequestration of autologous erythrocytes labeled with sodium chromate Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy spece Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy specimen confirmed the diagnosis of IPH. A diagnosis of IPH should be considered when children have iron deficiency anemia and pulmonary signs or symptoms. Organ sequestration studies may be helpful in equivocal cases.

Precocious puberty is a significant child health problem, especially in girls, because 95% of cases are idiopathic. Our earlier studies demonstrated that low-dose levels of manganese (Mn) caused precocious puberty via stimulating the secretion of luteinizing hormone-releasing hormone (LHRH). Because glial-neuronal communications are important for the activation of LHRH secretion at puberty, we investigated the effects of prepubertal Mn exposure on specific glial-derived puberty-related genes known to affect neuronal LHRH release. Abstract in english We hypothesized that chronic oral administration of the phosphodiesterase-5 inhibitor sildenafil could improve the exercise capacity and pulmonary hemodynamics in patients with pulmonary arterial hypertension (PAH) on the basis of previous short-term studies. We tested this hypothesis in 14 subjects with PAH, including seven patients with the idiopathic form and seven patients with atrial septal defects, but no other congenital heart abnormalities. Patients were subjected (more) to a 6-min walk test and dyspnea was graded according to the Borg scale. Pulmonary flow and pressures were measured by Doppler echocardiography. Patients were given sildenafil, 75 mg orally three times a day, and followed up for 1 year. Anomalies of the brain often cause foetal and neonatal death, and mental and physical retardation in pediatric group. The majority of disorders of brain development and phakomatosis are idiopathic, and most of them are not hereditary or familial. Ultrasonography plays the important role in screening foetal and neonatal brain, but after closure of fontanels it is difficult to find the acoustic window. CT has limited contrast resolution, and disadvantage exposing infant to ionizing radiation. It is helpful to demonstrate the presence of calcifications. The aim of this paper is to provide a general view of wave energy resource assessment. First, a review of the origin of waves and the transformation they undergo as they propagate towards the coast through waters of decreasing depth is presented. Following this, the wave and wave-energy parameters and the statistics required for resource characterization are described. The various types of wave data and their usefulness for the present purposes are summarised. A common methodology for assessment of the wave energy resource is developed. Finally, a general description of the global open ocean resource is presented.

This paper presents a case study in the design and implementation of a numerical weather prediction model on a supercomputer (CRAY-1). Following a historical introduction to the evolution of the model, the governing equations of the model are presented and the numerical solution of these forecast equations is described. A brief tutorial on the architecture of the CRAY-1 is presented with a discussion of how it affects the choice of algorithms and code design of the model. A summary of the advantages gained by use of the vector aspects of the CRAY-1 is included.

Section 2 discusses EPA`s cost-effectiveness methodology and identifies the pollutants included in the analysis. Abstract Reticulate pattern is one of the most important dermatological signs of a pathological process involving the superficial vascular networks. Vascular malformations, such as cutis marmorata congenita telangiectasia and benign forms of livedo reticularis, and sinister conditions, such as meningococcal meningitis or Sneddon's syndrome, can all present with a reticulate pattern. The clinical presentation and morphology is determined by the nature and extent of the underlying pathology and the involvement of a particular vascular network. This review has been divided into four instalments. In the present paper a review of the Dutch RAS programme is presented. The objectives and achievements of the programme are described. Special attention is given to a recent assessment of the international status of P and T that has been made for the Dutch authorities. The major conclusion of this assessment is that P and T is an important instrument in the management of nuclear waste to achieve the technical limits (ALARA) with respect to radiotoxicity of the waste inventory as well as radiological effects of the disposal. Focussed ion beam (FIB) technology has the advantage of being a maskless process compatible with UHV processing. This makes it attractive for use in in situ processing and has been applied to the fabrication of various mesoscopic structures. The present paper reviews these results whilst putting emphasis on in situ processing by a combined FIB and molecular beam epitaxy system. The typical performance of present FIB systems is also presented. In order to utilize the potential advantages of FIB processing, reduction of damage and improvement of throughput are important, and much effort has been devoted to developing processing techniques which require a reduced dose. In an ideal system the surface of the dish is perfectly parabolic (no contour flaws are present), and the sun rays fall on the dish surface parallel to the focal axis (no sun tracking flaws are present). These two conditions are not realizable in practice; in this case, the parabolic dish system is called real system. In this paper a procedure for analysing the flux distribution on the receiver internal surfaces is presented for a cavity receiver with a real parabolic dish collector. First, the theoretical principles are discussed. Then the effect of the inaccuracy in the collector and in the sun tracking on the insolation pattern is described using an example.

The diagnostic designs for the Laser Megajoule (LMJ) will require components to operate in environments far more severe than those encountered in present facilities. This harsh environment will be induced by fluxes of neutrons, gamma rays, energetic ions, electromagnetic radiations, and, in some cases, debris and shrapnel, at levels several orders of magnitude higher than those experienced today on existing facilities. The lessons learned about the vulnerabilities of present diagnostic parts fielded mainly on OMEGA for many years, have been very useful guide for the design of future LMJ diagnostics. The present and future LMJ diagnostic designs including this vulnerability approach and their main mitigation techniques will be presented together with the main characteristics of the LMJ facility that provide for diagnostic protection.

Methods of designing ultrasonic piezoelectric transducers in accord with a specified directivity pattern are analyzed and general formulas are presented for the design problem. As an example, the pressure distribution on the working surface of a circular piezoelectric transducer is determined that is capable of forming a narrow weakly divergent beam. Experimental results are presented with respected to ultrasonic fields that support the theoretical conclusions.

A simple and effective method is presented for the sizing and passage apportionment of multistream plate-fin heat exchangers. Methods for passage arrangement are discussed, and it is shown that a symmetric or mixed passage arrangement is more efficient than a segregated arrangement. A method for assessing the relative merits of various passage arrangements is presented. The use of the methods is illustrated through a worked example.

Resistance exercise has been widely reported to have positive rehabilitation effects for patients with neuromuscular and orthopaedic conditions. This paper presents an optimal design of magneto-rheological fluid dampers for variable resistance exercise devices. Adaptive controls for regulating the resistive force or torque of the device as well as the joint motion are presented. The device provides both isometric and isokinetic strength training for various human joints.

Experimental results of a search for the {Xi}{sub 3/2}(1860) cascade pentaquark state in data collected with the CDF 2 Detector in Run II at the Tevatron are presented. No evidence for these states in the neutral {Xi}{sup -}{pi}{sup +} and doubly charged {Xi}{sup -}{pi}{sup -} modes has been found. Preliminary upper limits on yields at 1862 MeV/c{sup 2} relative to the well established resonance {Xi}*(1530){sup 0} are presented.

In this paper, a new design methodology for determining the size, location, type and number of capacitors to be placed on a radial distribution system is presented. The objective is to minimize the peak power losses and the energy losses in the distribution system considering the capacitor cost. A sensitivity analysis based method is used to select the candidate locations for the capacitors. A new optimization method using a Genetic Algorithm is proposed to determine the optimal selection of capacitors. Test results have been presented along with the discussion of the algorithm.

Using the method of unitary transformation in combination with chiral effective field theory we derive the pion exchange contributions to the two-nucleon electromagnetic current. A formal definition of the current operator in this scheme and the power counting is presented. We discuss the implications of additional unitary transformations that have to be present to ensure the renormalizability of the one-pion exchange current. Further, we give explicit and compact results for the current in coordinate-space.

A Multi-Channel Algebraic Scattering (MCAS) theory is presented with which the properties of a compound nucleus are found from a coupled-channel problem. The method defines both the bound states and resonances of the compound nucleus, even if the compound nucleus is particle unstable. All resonances of the system are found no matter how weak and/or narrow. Spectra of mass-7 nuclei and of {}^{15}F, and MCAS results for a radiative capture cross section are presented.

The observation by Maday in 1974 that the Schmidt-Duffin solution to the optimal fin profile problem was flawed due to neglect of the slant height factor led to a new solution--actually, a demonstration of the ill-posedness of the question--by Snider. The present article proves mathematically that omission of this factor leads inevitably to underestimation of the heat transfer capability of any fin. Additionally, a perturbation scheme for estimating the correction for this effect is derived for configurations with low generalized Biot numbers, and a revised assessment of the performance of the Schmidt fin is presented.

This document deals with energy management in buildings. It presents the conditions for a good implementation of energy management. These conditions include the total involvement of the management and the training of the staff. The structure of this implementation is presented together with the tools for an efficient energy management. Calculations of total and angle-differential excitation cross sections for the electron impact excitation of lithium-like iron were performed with the R-matrix formulation. The alignments of the excited states 1s{sup 2}np {sup 2}P{sub 3/2} are also presented for n=3 and 4 along with the angular photon distribution from these states to the ground state. Relativistic effects were included in the present calculations by the Breit-Pauli Hamiltonian.

This paper describes the research and development activities in three areas of radiation processing, namely: food irradiation, medical product sterilization and wood plastic combination. Plans and efforts exerted to acquire a larger gamma source to augment our present 5,000 curie source are discussed. Cost estimates for a radiation facility are presented on the basis of the market potential of food irradiation and medical product sterilization. Existing local industries that can benefit from the adaptation of irradiation technology in their processing requirements are described.

Design tools that are currently available for use with microcomputers to assist in designing energy-efficient daylighted buildings are presented. The information presented was gathered as a part of a telephone survey to design tool developer conducted over a six month period. The matrix is but one of five categories of tools. The other four were nomographs, protractors/tables, minis, and main frame computer programs. Information is provided to encourage interested design professionals, educators and students to communicate directly with design tool representatives.

Helical tomotherapy presented many unique dosimetric challenges and solutions during the initial commissioning process, and some of them are presented. The dose calculation algorithm is convolution/superposition based. This requires that the energy fluence spectrum and magnitude be quantified. The methodology for doing so is described. Aspects of the energy fluence characterization that are unique to tomotherapy are highlighted. Many beam characteristics can be measured automatically by an included megavoltage computed tomography imaging system. This greatly improves data collection efficiency.

The tapeworm Spirometra mansonoides infects man worldwide, particularly in Asian countries. Rarely, the central nervous system is involved; such a case is presented here. In the total of 12 reported cases, including the case described, the worm presented clinically as a mass suspicious for neoplasm or chronic abscess cavity. Surgical removal was invariably curative in each case. Although infrequent, the possibility of tapeworm infection should be entertained in the evaluation of intracranial masses in patients who have visited exotic locales. PMID:2191089

The authors present a case in which primary Ewing's sarcoma of the right petrous pyramid in a 9-year-old girl showed no uptake on a /sup 99m/Tc-pertechnetate nuclide angiogram. Intense uptake was present on a /sup 99m/Tc-polyphosphate bone scan, but a static brain scan was only minimally abnormal. The diagnosis and treatment of Ewing's sarcoma are reviewed.

In a recent paper, Lowry (1981) described an architecture for a computer vision rectangular processor array that is suitable for VLSI implementation. In this paper the authors review that architecture, discuss extensions to it and present results of an array simulator applied to vision algorithms. They also present an algorithm for re-routing an array with bad processors into a working subset of the array, making it feasible to implement a large array on one wafer-sized chip. 7 references.

Pollutant emissions due to fossil fuel and biomass energy utilization in Turkey have been estimated. Standard emission factors were used for estimating the levels of particulate matter, sulfur oxides, nitrogen oxides, carbon monoxide, volatile organic compounds, and aldehydes. The results are presented for different fuels and energy-consuming sectors for the reference year 1987. Total Turkish emissions vs energy consumption are presented for every fifth year since. 1960. Free-electron lasers as scientific instruments are reviewed. The present status and future prospects are delineated with attention drawn to the size, complexity, availability, and performance capability of this new tool. The Free-Electron Laser (FEL) was proposed by John Madey in 1970 (1), although earlier work, relevant to the concept, had been performed by Motz (2) and by Phillips (3). Experimental demonstration was achieved by Madey, et. al. in 1975 and 1976 (4). Since that time, FELs of diverse configurations have been operated at several laboratories around the world. At present, FEL development is focused in two directions: in constructing reliable FELs for scientific research and in extending FEL capability to vacuum ultra-violet (VUV) and even shorter wavelengths. Aims: To assess the clinical characteristics and long-term outcome of patients with Zollinger-Ellison syndrome. Methods: Over a 20-year period, patients with Zollinger-Ellison syndrome were enrolled in a prospective trial evaluating the efficacy of lansoprazole. Following dose stabilization, patients were followed on a 6-monthly basis with interval history, physical examination, endoscopy with gastric biopsies, gastric acid analysis and laboratory studies. The thin windows for the Muon Ionization Cooling Experiment (MICE) liquid Absorber will be fabricated from 6061-T6-aluminum. The absorber and vacuum vessel thin windows are 300-mm in diameter and are 180 mm thick at the center. The windows are designed for an internal burst pressure of 0.68 MPa (100 psig) when warm. The MICE experiment design calls for changeable windows on the absorber, so a bolted window design was adopted. Welded windows offer some potential advantages over bolted windows when they are on the absorber itself. This report describes the bolted window and its seal. This report also describes an alternate window that is welded directly to the absorber body. The welded window design presented permits the weld to be ground off and re-welded. This report presents a thermal FEA analysis of the window seal-weld, while the window is being welded. Finally, the results of a test of a welded-window are presented.

We propose a quantum secure direct communication scheme based on non-orthogonal entangled pairs and local measurement. In this scheme, we use eight non-orthogonal entangled pairs to act as quantum channels. Due to the non-orthogonality of the quantum channels, the present protocol can availably prohibit from all kinds of valid eavesdropping and acquire a secure quantum channel. By local measurement, the sender acquires a secret random sequence. The process of encoding on the random sequence is identical to the one in one-time-pad. So the present protocol is secure. Even for a highly lossy channel, our scheme is also valid. The scheme is feasible with present-day techniques.

The present authors have previously developed a new method for Probabilistic Fracture Mechanics (PFM), which they call Recursive Distribution (RD) method. The method is based on the construction of the Lebesgue-Stieltjes measure through a deterministic mapping defining a crack growth process. In the present paper, its theoretical background is first discussed, and the Lebesgue decomposition of the measure is given. Then a numerical example of a Light Water Reactor (LWR)`s piping problem is solved by the present method, and the results are compared with those of the Monte Carlo (MC) method. In addition to leakage probability, a variation in stress cycles of the marginal distribution of an aspect ratio of a semi-elliptical surface crack is calculated, which will be used in a study on LBB evaluation.

Environmental radon monitoring activities at the DOE Fernald Environmental Management Project (FEMP) have been conducted extensively since the early 1980`s. Monitoring has been conducted at ambient concentration levels (< 1 pCi/L Rn-222), inside buildings, and at significantly elevated levels (hundreds of thousands pCi/L Rn-222) within the K-65 silo that store concentrated radium bearing wastes. The purpose of this paper/presentation is to present and discuss some of the difficulties encountered/solutions (e.g. reliability, detection limits, affects of environmental factors, data transfer, etc.) that have been discovered while taking measurements using both alpha track-etch passive integrating detectors and alpha scintillation real-time detectors. A short summary and conclusion section is provided following each topic presented.

Extraskeletal Ewing's sarcoma (ES) and primitive neuroectodermal tumor (PNET) are widely regarded as clinically and histologically identical tumors which consist of small blue round cells. Extraskeletal ESs/PNETs usually occur in the deep soft tissues of the paraspinal region, chest wall, or lower extremities. However, superficially located cases, so-called cutaneous ESs/PNETs, are exceedingly rare, and the vast majority of the reported cases present as a single small mass. We present magnetic resonance imaging (MRI) findings and clinical course of a unique case of primary cutaneous ES/PNET presenting as numerous huge masses with severe ulceration on them. (orig.)

Details of a 1 day conference designed to create more efficient interactions between the gas supply and electricity systems in Ontario and new models for gas supply and the electricity system were presented. Market efficiency, generators' perspectives on critical gas issues, an overview of the North American gas and an outline of issues concerning Ontario were discussed. A natural gas industry forecast and issues concerning marketers and producers were presented. The terms on which gas is bought and sold were discussed. Regulatory tools, cost allocation and issues relating to unbundling were also presented. Infrastructure needs were reviewed, as well as basic market structures. It was suggested that appropriate commercial service options need to be developed and approved, including cross-utility services.
