    Wund-D.A.CH. ist der Dachverband deutschsprachiger Fachgesellschaften, die sich mit den Thematiken der Wundbehandlung beschäftigen. Experten verschiedener Fachgesellschaften aus Deutschland, Österreich und der Schweiz haben nun einen aktuellen Konsens der Kompressionstherapie für Patienten mit Ulcus cruris venosum erstellt. In Europa ist das Ulcus cruris venosum eine der häufigsten Ursachen für chronische Wunden. Neben der konservativen und interventionellen Wund- und Venentherapie, ist die Kompressionstherapie die Basis der Behandlungsstrategien. Die Kompressionstherapie kann heute mit sehr unterschiedlichen Materialien und Systemen durchgeführt werden. Während in der Entstauungsphase insbesondere Verbände mit Kurzzugbinden oder Mehrkomponentensysteme zur Anwendung kommen, sind es anschließend überwiegend Ulkus-Strumpfsysteme. Eine weitere, bislang wenig verbreitete Alternative sind adaptive Kompressionsbandagen. Insbesondere für die Rezidivprophylaxe werden medizinische Kompressionsstrümpfe empfohlen. Durch die Vielzahl der heute zur Verfügung stehenden Behandlungsoptionen, kann für nahezu alle Patienten ein Konzept entwickelt werden, dass sich an den individuellen Bedürfnissen und Fähigkeiten orientiert und daher auch akzeptiert und durchgeführt wird. Die Kompressionstherapie ist für die Behandlung von Patienten mit Ulcus cruris venosum essentiell. In den letzten Jahren sind viele verschiedene Therapieoptionen verfügbar, die in den deutschsprachigen Ländern unterschiedlich angewendet oder durchgeführt werden. Daher soll dieser Expertenkonsens dazu beitragen, konkrete Empfehlungen für die praktische Durchführung der Kompressionstherapie von Patienten mit Ulcus cruris venosum darzustellen. © 2016 Deutsche Dermatologische Gesellschaft (DDG).     Eine Säule der kausalen Therapie bei Patienten mit Ulcus cruris venosum ist die Kompressionstherapie. Sie unterstützt die Abheilung, reduziert Schmerzen und Rezidive und steigert die Lebensqualität. Bislang existieren kaum wissenschaftliche Daten zu dem Versorgungsstand und fachspezifischem Wissen von Patienten mit Ulcus cruris venosum. Standardisierte Fragebögen wurden bundesweit in 55 Pflegediensten, 32 Arztpraxen, vier Wundzentren und -sprechstunden sowie einem Pflegetherapiestützpunkt von Patienten mit Ulcus cruris venosum bei Erstvorstellung anonym ausgefüllt. Insgesamt nahmen 177 Patienten (Durchschnittsalter 69,4 Jahre; 75,1 % Frauen) teil. Ein florides Ulcus cruris venosum bestand im Mittel 17 Monate. 31,1 % hatten keine Kompressionstherapie, 40,1 % Binden und 28,8 % Strümpfe. Bei der Bestrumpfung hatten 13,7 % Kompressionsklasse III, 64,7 % Kompressionsklasse II und 19,6 % Kompressionsklasse I. 70,6 % legten die Strümpfe nach dem Aufstehen an, 21,1 % trugen sie Tag und Nacht. 39,2 % bereiteten die Strümpfe Beschwerden. Lediglich 11,7 % hatten eine An- und Ausziehhilfe. Die Binden wurden im Mittel 40,7 Wochen getragen und bei 69 % nicht unterpolstert. Bei 2,8 % wurde der Knöchel- und Waden-Umfang zur Erfolgskontrolle gemessen. Venensport machten 45,9 %. Ein Drittel hatte keine Kompressionsversorgung, obwohl diese eine Basismaßnahme der Therapie des Ulcus cruris venosum ist. Zudem ist deren korrekte Auswahl und Anwendung angesichts der langen Bestandsdauer der Ulzerationen zu hinterfragen.     Full Text Available Ulcus vulvae acutum is a rare clinical condition characterized by the presence of multiple acute painful genital ulcers of non-venereal origin associated with systemic symptoms in young women. The aetiopathogenesis of the disease is not fully understood, although recent reports have associated it with the Epstein–Barr virus. Diagnosis is difficult and generally made by exclusion after venereal diseases, and autoimmune, inflammatory, traumatic, and neoplastic causes. We describe a case of adolescent female with an episode of ulcus vulvae acutum associated with infectious mononucleosis. The diagnosis was supported by the clinical symptoms, elevated circulating levels of liver enzymes, positive EBV serology, cervical and inguinal lymphadenomegaly, and hepatosplenomegaly. The patient presented a history of aphthous stomatitis. Negative Pathergy test and the absence of any other related symptoms allowed us to exclude the Behçhet syndrome. Lesions healed with no sequelae or recurrences.

    The goal of this work is a correlation of the radiological and endoscopic findings of a university clinic in cases of stomach and duodenal ulcers. The sensitivity of the radiological examinations with Ulcus ventriculi, respectively duodeni, was 89%, respectively 91%; the specificity was 74%, respectively 85%. From these results suggestions were made for the use of endoscopy with radiologically diagnosed ulcers of the duodenum or stomach. Whereas the endoscopy is superior for the recognition of florid ulcers in a deformed Bulbus duodeni, radiology is superior in determining sinus developments, stenotic areas and the extent of an evacuation disturbance.     Full Text Available The proposed treatment of ulcus decubitalis ulcers III (an area of more than 2 cm2 and stage IV in 31 patients with consequences of severe traumatic brain injury (TBI and cerebral stroke during their rehabilitation in Federal Research and Clinical Center of Intensive Care Medicine and Rehabilitology. Used has developed a unique technique and customary for this category of patients treatment. In the control group of 34 patients with ulcus decubitalis II and III (area less than 2 cm2 stages were treated in accordance with medical standards. In the study group, consisting of 31 patients, the use of the methodology obtained good and satisfactory results in 100% of the study group (compared with 31.6% of patients had complete healing of ulcers, 35% of patients healing 70–90% by volume of the wound, 33.4% of the healing of 30–70% by volume of the wound.

    Iron lack anemia due to chronic blood loss was diagnosed in a 12-year-old dog. Clinical abnormalities included weakness and episodic vomiting. Typical hematological abnormalities were moderate regenerative anemia (Hct 21 %) and microcytosis (MCV 39 fl.). Chronic occult blood loss in adult dogs most commonly occurs in the gastrointestinal tract, associated with ulcus or neoplasia. Possible diagnostic steps include radiographs, abdominal ultrasound, gastroduodenoscopy, and exploratory laparotomy.     By the help of successes achieved in Japan in the control of gastrocarcinoma, tasks, possibilities and limits in X-ray diagnostics are outlined, strategically holding hitherto a key position in the recording of patients with early carcinoma of the stomach. X-ray symptoms, the classification of tumor-finding, X-ray examination-methods, their relation to gastroscopy as well as the radiation dose and problems of economy of X-ray examinations are discussed.     Full Text Available Background: Indonesia is a country with the world’s biggest potential and producer of sea cucumbers. Golden sea cucumber contains glicosaminoglycans, such as heparan sulphate and chondroitin sulphate, which could have a positive implication on wound healing process. This acceleration of wound healing process could be observed through the increasing of lymphocytes on ulcus traumaticus. Purpose: This study aims to analyze the effects of golden sea cucumber extract on the number of lymphocytes during the healing process of traumatic ulcer on Wistar rat’s oral mucous. Method: Golden sea cucumber extrat was made with freeze-dried method, and then gel was prepared using PEG 400 and PEG 4000 solvent. Twenty male rats with mucosal ulcus made were divided into a control group and three treatment groups with 20%, 40% and 80% golden sea cucumber extracts. All samples were euthanized on day 4 and then a preparation for histopathological examination was made to examine the number of lymphocytes. Result: The biggest number of lymphocytes was found in the treatment group with 40% golden sea cucumber extract, while the lowest one was found in the control group. The results of one way Anova test then showed a significant difference between the control group and the treatment groups. And, the results of Tukey HSD showed a significant difference between the control group and the treatment group with 40% golden sea cucumber extract. Conclusion: It can be concluded that 40% golden sea cucumber (Stichopus hermanii extract can increase the number of lymphocytes during the healing process of traumatic ulcer on Wistar rat’s oral mucous.

    99m Tc HIDA was put into use first under standardized conditions in the framework of routinely carried out stomach secretion analyses in order to study the importance of bile reflux in stomach disorders. With this procedure a simple possibility is given in the area of stomach secretion analysis to test if there is a bile reflux to the stomach. Patients with fresh duodenal ulcers had significantly higher reflux rates than those with chronic Ulcus duodeni. After selective proximal vagotomy with submucal pyloro plastic the reflux rates sank in comparison to those before the operation.     We report on applications of high-resolution clinical multiphoton tomography based on the femtosecond laser system DermaInspectTM with its flexible mirror arm in Australia, Asia, and Europe. Applications include early detection of melanoma, in situ tracing of pharmacological and cosmetical compounds including ZnO nanoparticles in the epidermis and upper dermis, the determination of the skin aging index SAAID as well as the study of the effects of anti-aging products. In addition, first clinical studies with novel rigid high-NA two-photon 1.6 mm GRIN microendoscopes have been conducted to study the effect of wound healing in chronic wounds (ulcus ulcera) as well as to perform intrabody imaging with subcellular resolution in small animals.

    This report deals with a basal cell epithelioma, partially adenoid and partially morphea-like in structure, which despite intensive X-ray treatment relapsed constantly and which finally developed into an ulcus terebrans. Approximately 13 years after the primary tumor had developed (located on the left wing of the nose) both a lymphogenic and a hematogenic formation of metastases occurred with a subsequent exitus letalis 4 months later. Besides the metastases of the skin, there were multiple metastases in the lymph nodes, vertebral column, ribs, spleen, liver, stomach, pleura, and peritoneum as well as in the myocard of both ventricles and in the perimysium of the skeletal muscles. Their histological structure was similar to a partly adenoid, partily morphea-like basal cell epithelioma. The possible influence of X-ray treatment on the tumor tissue in way of benignity or malignancy is discussed in view of relevant literature on this topic. Approximately 13 years after the primary tumor had developed (located on the left wing of the nose) both a lymphogenic and a hematogenic formation of metastases occurred with a subsequent exitus letalis 4 months later. Besides the metastases of the skin, there were multiple metastases in the lymph nodes, vertebral column, ribs, spleen, liver, stomach, pleura, and peritoneum as well as in the myocard of both ventricles and in the perimysium of the skeletal muscles. Their histological structure was similar to a partly adenoid, partily morphea-like basal cell epithelioma. The possible influence of X-ray treatment on the tumor tissue in way of benignity or malignancy is discussed in view of relevant literature on this topic. The alteration of basal cell epitheliomas into the socalled transitional epitheliomas is also analyzed.

    Full Text Available Every year, about 2.5 million people are affected from burns in the world. In our country there is no reliable database related to this subject. There are ongoing studies about the epidemiology of burns in Turkey. After burn injury had represent many various complications, such as myocardial infarction, cardiac deficiency, acute hypertension, endocarditis, thromboembolism, pulmonary edema, pneumonia, respiratuar failure, renal failure, gastric ulcus, ileus, sepsis, coagulopathy and anemia. Such complications can preventable or treatable. In this respect, preventive management in the first step burn treatment had very importantly in burn cases. Skin is a barrier which protects evaporative heat loss. In cases of acute burn, hypothermia occurs related to skin loss. For these cases, care must be taken to keep the patient warm. In addition fluid resuscitation is very important in these cases. Furthermore, the damaged tissues are highly susceptible to infection in burned patients. Burn care and rehabilitation includes challenging and complex procedures. Briefly, treatments of burn cases require a multidisciplinary and meticulous approach.

    Mucosa associated lymphoid tissue (MALT) lymphomas are a group of non- Hodgkin"s lymphomas of low malignancy degree. The most frequent location is the gastrointestinal tract. Its primary pulmonary presentation is unusual and heterogeneous from point of view radiological. Woman 61 years old with antecedents of vitiligo, gastric ulcus, cirrhosis by VHC, that go into the hospital by sudden disnea, thoracic paint with pleural characterises and fever of 38.5 degrees C, Her thorax radiography and thoracic TAC showed nodes that affect to different pulmonary lobes. The cytology by PAAF confirms their malignant nature. In subsequent radiological controls it was notice the nodels took away completely and returns in different pulmonary place in each recurrence. The presentation like fluctuant pulmonary nodes is exceptional in a MALT lymphoma. It was described a higher incidence of VHC infection and tumour. The evidence of chronic hepatitis by virus C disease, and local chronic inflammatory process as well as autoimmune disorders may be considerate like a factor that contribute to MALT lymphoma.

    For diagnosing pathologies of the peripheric vessels or of the lymphatic system, albumin aggregates marked with sup(99m) technetium (MAA) or antimonocolloids were used. The radionuclide venography is useful in diagnosing thrombosis, venous insufficiency, and permits a high degree of accuracy, comparable with that of phlebography. In the most cases the radionuclide lymphoscintigraphy is required for the diagnosis of lymphoedema and allows by distribution analysis an adequate morphological representation of the lymphatic ducts and glands. Contrary to lymphography, lymphoscintigraphy presents a completely physiological radionuclide transport and does not cause any reactive lymphatic node hyperplasia by contrast media. Increased capillary perfusion is found in collateral vessel formation in cases of arterial obstruction and ischaemic ulcus, whereas hypoperfusion is seen in obstructions of the larger arteries having no collateral vessels, and in bad circulatory states. Particularly in diabetics with microangiopathy, a peripheral distribution with hyperfusion of the skon presents itself as piece-meal necrosis. The importance and relevance of the perfusion scans of the extremities for the diagnostic assessment within the field of resection surgery is explained.

    Full Text Available Objetivos: analizar la evolución, a lo largo del periodo 1999-2005, de las siguientes variables de los pacientes ingresados por hemorragia digestiva (HD en una Unidad de Sangrantes: etiología, patología de base, consumo de AINE/anticoagulación y mortalidad. Material y métodos: durante el periodo 1999-2005 se ha estudiado la evolución de las siguientes causas de HD que requirieron ingreso en la Unidad de Sangrantes: ulcus duodenal (UD, ulcus gástrico (UG, hipertensión portal (HPT y otros. De igual forma se ha analizado la evolución en el porcentaje de enfermos ingresados con patología de base, consumo de fármacos AINE/anticoagulantes y mortalidad. Resultados: se han incluido 1.611 pacientes en el estudio con una edad media de 60,45 años (59,7-61,2, 76,41% hombres (74,3-78,5. La UD fue responsable del 22,20% de episodios (20,2-24,3, la UG del 18,40% (16,6-20,4 y la HPT del 33,60% (31,3-36,0. De forma global realizaban tratamiento con AINE el 34,5% (32,6-37,3, anticoagulación el 7,1% (6,0-8,6, presentaban patología de base el 72,6% (70,4-74,8 y la mortalidad global fue del 6,27% (5,16-7,59.     Full Text Available Objetivos: describir los cambios que se han producido en las características de los pacientes aquejados de hemorragia digestiva alta no varicosa y en la epidemiología de la misma. Material y métodos: mediante un estudio prospectivo observacional de la incidencia y causas de la hemorragia digestiva no varicosa en el área de salud correspondiente al Hospital Virgen de las Nieves de Granada, España, comparando tres periodos: durante 1985 (grupo 1: 284 pacientes; otro en 1996 (grupo 2: 259 pacientes; y en 2006 (grupo 3 : 291 casos. Resultados: la incidencia estudiada es de 71/100.000 habitantes al año en el grupo 1, 64 en el 2 y 66 en el 3. La edad media en 1985 fue de 57,4 años; en 1996: de 59,6 años; y en 2006 de 62,38. En todos los grupos evidenciamos una mayoría de hombres (75,4, 69,5 y 72,2% respectivamente. Entre las causas destaca: ulcus duodenal (1: 40,5%; 2: 43,2%; 3: 40,5%, úlcera gástrica (1: 24,3%; 2: 30%; 3: 18,9%, LAMG (1: 15,3%; 2: 8,9%; 3: 9,6%, neoplasia (1: 1,7%; 2: 1,9%; 3: 5,2%, lesiones vasculares ( 1: 0,5%; 2: 1,5%; 3: 9,3%.   15. Vacuum assisted closure in vascular surgery.

    Vacuum assisted closure (VAC-therapy) is a well established method in nearly all surgical disciplines. The aim is to present the efficiency of vacuum assisted closure in the treatment of acute and chronic wounds in patients admitted in the department of vascular surgery. Within the year 2008 there were 59 patients (44 men, 15 women) treated with VAC therapy in our Department of Vascular surgery (Landshut, Germany). VAC was used 22x (37.28 %) in therapy of ulcus cruris (venous, arterial, mixed genesis), 15x (25.42%) in patients with diabetic foot syndrome, 12x (20.33%) in secondary healing wounds and infected wounds, 5x (8.47%) in wounds after several injuries and soft skin tissue infections and 5x (8.47%) in wound infections connected with vascular graft infections after vascular revascularization. VAC therapy seems to be very effective in the management of patients with venous ulcers, especially after a proper surgical treatment (100%), patients with soft skin tissue infections (100%) and secondary healing wounds (100%) especially in combination with MESH-Grafting. In patients with diabetic foot syndrome (80%) and peripheral arterial occlusive disease (72.7%), an evaluation of peripheral blood perfusion and revascularization prior to VAC therapy is often necessary. Although VAC was used 5x in the therapy of infected vascular grafts, successful preservation of infected graft material was observed in only one case (infection of PTFE femoro-popliteal bypass graft). Vacuum assisted closure in vascular surgery proved to be simple and efficient method in therapy of acute and chronic wounds. The efficiency of VAC systems in therapy of infected graft material after revascularization needs further studies (Tab. 3, Ref. 10).

  16. [Influence of early kinesitherapy on rehabilitation postoperative recovery in case of low extremity operations].

    Arteriosclerosis is a disease effecting large elastic, elastic muscular and large muscular arteries. The primary pathological changes are found inside the arteries. Salient feature is atherosclerotic plaque. The main purpose of this paper is to determine whether early kinesitherapy with verticalization and mobilization can give better rehabilitation results with patients after lower extremities operation, in comparison to patients which were not mobilized early in this stage. In this paper we have analyzed two groups of 40 patients that were operatively treated for circulation obstruction to lower extremities. The first, tested group was treated with early kinesiotherapy with early verticalization and mobilization. The second, controlled group was treated with kinesitherapy in bed. All the patients were analyzed according to their gender, age, primary diagnosis, postoperative complications, as well as capabilities for everyday activities. For the evaluation of the everyday life activities we used the Barthel index at hospital admission and discharge. Both groups of patients were equable in gender and age. The most common age group within the tested groups was between 61-70 for males and 51-60 for woman.The most common type of obstruction in both groups was occlusion of femoral artery. The complications which occurred among the tested patients were thrombosis, ileus and contracture in 7.5% of cases, while 85% of patients did not have any complications. Within the controlled group, decubitus occurred in 7.5% of cases, as well as gangrene and ileus. Ulcus occurred in 5% of patients, thrombosis in 10%, and contracture in 4% of cases. 40% of patients in this group were without complications. Based on the results stated above we can conclude that early kinesitherapy with verticalization and mobilization of patients in postoperative phase after the treatment of lower extremities obstruction, have shown much better results in comparison to the patients which were only

Untuk mendiagnosis ulkus dengan tepat, dokter gigi harus mengevaluasi riwayat dan gambaran klinis secara detil dan jika lesi tersebut diduga disertai infeksi lainnya seperti fungal, bakteri atau virus maka diperlukan tes mikrobiologi atau serologi. Salah satu terapi awal jika diketahui adanya keterlibatan fungal dapat digunakan nystatin untuk mendukung pemulihan dan perbaikan jaringan epitel pada ulkus traumatic tersebut. Tujuan: Laporan kasus ini bertujuan untuk menekankan pentingnya pemeriksaan mikrobiologi pada kasus ulkus traumatic yang diduga disertai infeksi fungal. Kasus: Seorang anak perempuan usia 12 tahun datang ke klinik Ilmu Kedokteran Gigi Anak Fakultas Kedokteran Gigi Universitas Indonesia pada tanggal 9 Juni 2011 diantar ibunya. Pasien dengan riwayat geographic tongue datang dengan keluhan terdapat luka di bagian tengah lidah. Diagnosis utama adalah ulkus traumatic yang disertai infeksi fungal. Penegakan diagnosis ditetapkan dari hasil pemeriksaan swab. Tatalaksana kasus: Kasus ulcus traumatic ini diatasi dengan Dental Health Education, oral profilaksis, pemberian resep dan instruksi pemakaian nystatin. Terjadi pemulihan dan perbaikan epitel mukosa lidah setelah penggunaan nystatin. Kesimpulan: Pemeriksaan mikrobiologi penting dilakukan untuk menegakkan diagnosa ulkus traumatic yang diduga disertai infeksi fungal. Pengobatan yang tepat seperti pemberian nystatin dapat diberikan pada kasus ulkus traumatic yang disertai infeksi fungal.
