Narrative medicine is one of medicine's most important internal reforms, and it should be a critical dimension of healthcare debate. Healthcare reform must eventually ask not only how do we pay for healthcare and how do we distribute it, but more fundamentally, what kind of healthcare do we want? It must ask, in short, what are the goals of medicine? Yet, even though narrative medicine is crucial to answering these pivotal and inescapable questions, it is not easy to describe. Many of its core claims go against the grain of common sense thinking about medicine. This article argues that the best way to understand narrative medicine is to tell a story that puts its emergence in historical context. PMID:21053060

The New Yorker Richard Feynman, well known to all physicists for his diagrams. Eleven vivid posters have recently been hung along the Theoretical Physics corridor on the first floor of Building 53. They tell the story of a century of physics, between 1900 and the end of the nineteen-nineties, the century that gave us atomic and quantum physics and the great breakthroughs that have rocked our understanding of the world and the universe. The posters are a gift from the American Physical Society to the CERN Library. As they were designed for high school and university book collections in the US, they necessarily have a somewhat American view of the history of science. But it is still instructive to revisit contemporary physics through its great figures, such as Marie Curie, Stephen Hawking, and Richard Feynman, not to mention Albert Einstein and many others. Why not take the time for a little trip down this particular stretch of Memory Lane?

In this paper the authors describe the ways poststructuralist discourses assisted us to reading and reflection on data collected as part of a traditional survey-style research study. The study--generated in response to widespread concern about falling male enrolment rates at the authors' regional university--focused upon factors affecting rural male school-leavers' decisions about postschool destinations, and upon these students' attitudes to higher education. In the paper that follows, the authors contest the dominant discourses that are frequently drawn on to explain differences in male and female higher education enrolment trends. In the mid-seventies Kevlar was participating in only ten market segments and less than fifty specific applications, but today, it is in more than twenty market segments, serving more than two hundred applications, and continued growth is anticipated. The fusion breeder is a fusion reactor designed with special blankets to maximize the transmutation by 14 MeV neutrons of uranium-238 to plutonium or thorium to uranium-233 for use as a fuel for fission reactors. Breeding fissile fuels has not been a goal of the US fusion energy program. This paper suggests it is time for a policy change to make the fusion breeder a goal of the US fusion program and the US nuclear energy program. The purpose of this paper is to suggest this policy change be made and tell why it should be made, and to outline specific research and development goals so that the fusion breeder will be developed in time to meet fissile fuel needs.

Radionuclide X-ray fluorescence analysis (RXRFA), as a non-destructive method, relatively simple and allowing measurements to be carried out in situ, is an excellent tool in research on various objects of art. A range of artefacts have been investigated by our laboratory, in part for the purposes of history of art and in part as a basis for restoration works - medieval frescoes in some Czech castles and churches, metal sculptures and objects of applied art, paints and inks of old manuscripts, paintings. Some of these are among the most valuable monuments in the Czech cultural heritage. The contribution of the results of the tests to the investigation of their 'life story' and, in some cases, to their conservation, is not negligible. Later additions and repairs can be recognised, and materials and technologies that are close to their historic counterparts can be used in restoration work.

The uncertainty of the elemental analysis is one of the major factors governing the utility of on-line Prompt Gamma Neutron Activation Analysis (PGNAA) in the blending and sorting of bulk materials. In this paper, a general method applicable to Gamma spectra processing is presented and applied to PGNAA in mineral industry. Based on the Fourier transform of spectra and their de-correlation in the Fourier space (the improvement of the conditioning of the correlation matrix), processing of overlapping of characteristic peaks minimizes the propagation of random errors, which optimizes the accuracy and decreases the detection limits of elemental analyses. In comparison with classical methods based on the linear combinations of relevant regions of spectra the improvement may be considerable, especially when several elements are interfering. The method is applied to four case stories covering both borehole logging and on-line analysis on conveyor belt of raw materials.

The first 750 MW block of the new South Humber Bank gas-fired combined cycle power plant, sited about 300 km north from London, was officially commissioned in September 12th 1997. The project has been a major success story for Imatran Voima Oy (IVO), which developed the concept and managed the project and owns 22.5 % of the plant; and ABB Power Plants Limited, responsible for the turnkey delivery. IVO Generation Services (UK) is responsible for the running and maintenance of the facility. IVO also manages two other existing plants in Britain, at Brigg (240 MW) and Peterborough (360 MW).

Sensor has been developed for the measurement of seismic wave rotary components. The vertical axis rotary component sensor is constituted of four L22Ds, similar to each other in performance, fixed in a ring formation on a metal bed. The ring is 22cm in diameter, and the four coils are connected so that their outputs will be added up. A horizontal axis rotary component sensor is built in the similar way. The rotary component sensors are all connected to a spectrum analyzer for the measurement of micromotions. It is found that the four-L22D combination can detect a rotary component of 335mV/deg/s. The sensor can detect both vertical and horizontal components with roughly the same sensitivity. A ring-form combination of conventional electromagnetic seismometers, though only as a temporary expedient, can perform high-resolution detection of rotary components and determine the absolute values of earthquakes or noise levels. We calculate the light `glueball' mass spectrum in N_f=2 lattice QCD using a fermion action that is non--perturbatively O(a) improved. We work at lattice spacings a~0.1 fm and with quark masses that range down to about half the strange quark mass. We find the statistical errors to be moderate and under control on relatively small ensembles. We compare our mass spectrum to that of quenched QCD at the same value of a. Whilst the tensor mass is the same (within errors), the scalar mass is significantly lighter in the dynamical lattice theory, by a factor of ~0.84 +/- 0.03. We discuss what the observed m_q dependence of this suppression tells us about the dynamics of glueballs in QCD. We also calculate the masses of flux tubes that wind around the spatial torus, and extract the string tension from these. As we decrease the quark mass we see a small but growing vacuum expectation value for the corresponding flux tube operators. This provides clear evidence for `string ...

The Army Reactor Area (ARA) II was constructed in the late 1950s as a test site for the Stationary Low Power Reactor No. 1 (SL-1). The SL-1 was a prototype power and heat source developed for use at remote military bases using a direct cycle, boiling water, natural circulation reactor designed to operate at a thermal power of 3,000 kW. To which idea of truth may medicine refer? Evidence-based medicine (EBM) is rooted in the scientific truth. To explain the meaning and to trace the evolution of scientific truth, this article outlines the history of the Scientific Revolution and of the parable of Modernity, up to the arrival of pragmatism and hermeneutics. Here, the concept of truth becomes somehow discomfiting and the momentum leans towards the integration of different points of view. The fuzzy set theory for the definition of disease, as well as the shift from disease to syndrome (which has operational relevance for geriatrics), seems to refer to a more complex perspective on knowledge, albeit one that is less defined as compared to the nosology in use. Supporters of narrative medicine seek the truth in the interpretation of the patients' stories, and take advantage of the medical humanities to find the truth in words, feelings and contact with the patients. Smoke management in buildings during fire events often uses mechanical ventilation systems to maintain smoke layer elevation above a safe evacuation path. Design of these systems requires accurate correlations for the smoke production rate of the buoyant fire plume. One design issue is the smoke production rate of fire plumes which spill out from a fire compartment, under a balcony and up through an atrium or other large volume. Current engineering correlations for these balcony spill plumes are based on a combination of one-tenth scale test data and theoretical analysis. Questions have arisen over the suitability of these correlations for real-scale designs. A combined program of full-scale experimentation and CFD modeling is being conducted to analyze the accuracy of these correlations. This case study, conducted from an interpretive paradigm, illuminates contextual factors related to the tutor experience when senior undergraduate dental hygiene students served as tutors for beginning undergraduate dental hygiene students, or sophomores, in a 1-semester, 2-hour long problem-based learning (PBL) course in a Baccalaureate Dental Hygiene (BDH) curriculum during the spring semester of 2008. Data were collected using various sources and methods. Six tutors and three administrators were interviewed, tutees completed an anonymous questionnaire, the tutorial process and tutor training sessions were observed, and related documents were examined. Iodine deficiency is a risk factor for the growth and development of up to 800 million people living in iodine deficient environments throughout the world. The effects on growth and development, called the iodine deficiency disorders (IDD), comprise goiter, stillbirths and miscarriages, neonatal and juvenile thyroid deficiency, dwarfism, mental defects, deaf mutism, and spastic weakness and paralysis, as well as lesser degrees of loss of physical and mental function. All these effects are due to inadequate thyroid hormone production because iodine is an essential constituent of the thyroid hormone. In the West, IDD has been largely eliminated by the addition of iodine to the diet through iodized salt or through changes in food distribution and technology. IDD still persists in certain areas of Europe where these dietary changes have not occurred. The data center in this study had a total floor area of 8,580 square feet (ft{sup 2}) with one-foot raised-floors. It was a rack lab with 440 racks, and was located in a 208,240 ft{sup 2} multi-story office building in San Jose, California. Since the data center was used only for testing equipment, it was not configured as a critical facility in terms of electrical and cooling supply. It did not have a dedicated chiller system but served by the main building chiller plant and make-up air system. Additionally, it was served by a single electrical supply with no provision for backup power. The data center operated on a 24 hour per day, year-round cycle, and users had all hour full access to the data center facility. The study found that data center computer load accounted for 23% of the overall building electrical load, while the total power consumption attributable to the data center including allocated cooling load and lighting was 30% of the total facility load. ...

It surveys the impact of good and bad politics on the attainment of Meaningful Access to education with special focus on Nigeria's Universal Basic Education (UBE) programme. Good politics is to be likened to what the French call "la politique au sense noble du terme" (politics in the noble sense of the term--or statesmanship) while bad politics is to be likened to a "politique politicienne" (mere divisive politicking, or politics in its raw form). The compositions of the Earth's crust and mantle, and those of the Moon and Mars, are relatively well known both isotopically and elementally. The same is true of our knowledge of the asteroid belt composition, based on meteorite analyses. Remote measurements of Venus, the Jovian atmosphere, and the outer planet moons, have provided some estimates of their compositions. The Sun constitutes a large majority, > 99%, of all the matter in the solar system. The elemental composition of the photosphere, the visible 'surface' of the Sun, is constrained by absorption lines produced by particles above the surface. Abundances for many elements are reported to the {+-}10 or 20% accuracy level. However, the abundances of other important elements, such as neon, cannot be determined in this way due to a relative lack of atomic states at low excitation energies.
