MRI (Magnetic resonance imaging) has been widely used in the diagnosis of primary carpal tunnel syndrome (CTS). However, it has had limited clinical application in diagnosing persistent or recurrent CTS. We aimed to investigate the efficacy of this imaging modality in patients who had previously undergone open carpal tunnel release without relief of symptoms, and assess the correlation of MRI with intra-operative findings upon re-exploration. MRI studies were performed on 17 wrists (16 patients) presenting with recurrent/persistent symptoms and signs of CTS in whom repeat nerve conduction studies were also performed. The penetrance of Leber's hereditary optic neuropathy (LHON) in families with primary mitochondrial DNA (mtDNA) mutations is very complex. Matrilineal and nuclear genetic background, as well as environmental factors, have been reported to be involved in different affected pedigrees. Here we describe two large Chinese families that show a striking difference in the penetrance of LHON, in which 53.3% and 15.0% of members were affected (P < 0.02), respectively. Analysis of the complete mtDNA genome of the two families revealed the presence of the primary mutation G11778A and several other variants suggesting the same haplogroup status G2a. Abstract Hereditary primary hyperparathyroidism (HPT) may develop as a solitary endocrinopathy (FIHP) or as part of multiple endocrine neoplasia Type 1, multiple endocrine neoplasia Type 2A, or hereditary HPT-jaw tumor syndrome. Inactivating germline mutations of the tumor suppressor gene CDC73 account for 14 and 50% of all FIHP and HPT-JT patients, respectively, and have also been found in almost 20% of apparently sporadic parathyroid carcinoma patients. Although more than 60 independent germline mutations have been described, to date no rearrangement affecting the CDC73 locus has been identified. This paper analyses the effectiveness of non-formal schools for working children in Jalandhar, Punjab, India, in mainstreaming child labourers into the formal education system through incentivised, informal schooling. Using a family fixed effects model and sibling data as an equivalent population comparison group, I find that the non-formal schools effectively provide an alternative to formal primary education and also show high success rates of mainstreaming and maintaining children into post-primary education relative to the control group. I find that the children within the non-formal schools are 40.47-50.07% more likely to still be studying relative to the sibling-inclusive control group, and have 1.976 to 3.389 years less of a gap in educational attainment. Hereditary hemorrhagic telangiectasia (Osler-Rendu-Weber disease) is a genetic disorder with autosomal dominance, variable penetrance, and an estimated prevalence of 1/10,000 inhabitants in France. Diagnosis is based on clinical criteria including epistaxis, telangiectasia, visceral manifestations, and familial occurrence. Pulmonary arteriovenous malformations, present in 15-33% of patients, are its primary visceral complications. The disease may be revealed by infectious and ischemic neurological manifestations due to paradoxical embolism. The high frequency of neurologic complications even in asymptomatic patients justifies systematic screening for pulmonary arteriovenous malformations. Treatment of these malformations is based on percutaneous transcatheter coil embolization of the feeding artery. Pulmonary arterial hypertension is rare in this disease. The objective of this study is to determine the efficacy of the various conservative treatment options for relieving the symptoms of CTS. Computer-aided searches of MEDLINE (1/1966 to 3/2000), EMBASE (1/1988 to 2/2000) and the Cochrane Controlled Trials Register (2000, issue 1) were conducted, together with reference checking. Purpose: To evaluate the outcomes of patients with Ewing's sarcoma family of tumors (ESFT) treated with modern radiotherapy techniques with MRI along with optimal chemotherapy. Methods and Materials: The records of all 60 patients with ESFT who received radiation to the primary site between 1990 and 2004 were reviewed. All patients received chemotherapy, including vincristine, doxorubicin, cyclophosphamide, ifosfamide, and etoposide. Radiation was used as the sole modality for local control in 31 patients and was given either before (n = 3) or after surgical resection (n = 26) in the remainder. All patients had MRI and CT scan-based treatment planning, and 43% received intensity-modulated radiation therapy. Ewing's sarcoma is a small round-cell tumor typically arising in the bones, rarely in soft tissues, of children and adolescents. Ewing's sarcoma has retained the most unfavorable prognosis of all primary musculoskeletal tumors. Prior to the use of multi-drug chemotherapy, long-term survival was less than 10%. The development of multi-disciplinary therapy with chemotherapy, irradiation, and surgery has increased current long-term survival rates in most clinical centers to greater than 50%. In addition, the preferred method of tumor resection has changed; limb salvage has nearly replaced amputation of the affected limb. Limb salvage procedures can be performed in place of amputation without compromising patient survival rates. A one parameter family of iterative methods for the simultaneous approximation of simple complex zeros of a polynomial, based on a cubically convergent Hansen-Patricks family, is studied. We show that the convergence of the basic family of the fourth order can be increased to five and six using Newtons and Halleys corrections, respectively. Since these corrections use the already calculated values, the computational efficiency of the accelerated methods is significantly increased. Further acceleration is achieved by applying the Gauss-Seidel approach (single-step mode). Familial hypocalciuric hypercalcaemia (FHH) is an autosomal dominant disorder caused by inactivating mutations in the calcium sensing receptor (CaSR) gene; it is characterised by lifelong and usually asymptomatic hypercalcaemia. Establishing the correct diagnosis is important because surgery can be curative in HPT, but ineffective in FHH. Attention-deficit hyperactivity disorder (ADHD) has been shown to be familial and heritable, in previous studies. As with most psychiatric disorders, examination of pedigrees has not revealed a consistent Mendelian mode of transmission. The response of ADHD patients to medications that inhibit the dopamine transporter, including methylphenidate, amphetamine, pemoline, and bupropion, led us to consider the dopamine transporter as a primary candidate gene for ADHD. If fourth family condensates are responsible for electroweak symmetry breaking then they may also break approximate global symmetries. Among the resulting pseudo-Goldstone bosons are those that can have diquark quantum numbers. We describe the variety of diquarks and their decay modes, and we find aspects that are particular to the fourth family framework. Spectacular signatures at the LHC appear and are explored for color sextet diquarks with 600 GeV mass. We consider a simple search strategy which avoids diquark reconstruction. We also consider 350 GeV mass diquarks that are accessible at the Tevatron.

Generalized linear models (GLMs), as defined by J. A. Nelder and R. W. M. Wedderburn (1972), unify a class of regression models for categorical, discrete, and continuous response variables. As an extension of classical linear models, GLMs provide a common body of theory and methodology for some seemingly unrelated models and procedures, such as the logistic, Poisson, and probit models, that are increasingly used in family studies. This article provides an overview of the principle and the key components of GLMs, such as the exponential family of distributions, the linear predictor, and the link function. To illustrate the application of GLMs, this article uses Canadian national survey data to build an example focusing on the number of close friends among older adults. The article concludes with a discussion of the strengths and weaknesses of GLMs.

In accordance with federal legislation, the U.S. Department of Energy (DOE) has conducted a project to demonstrate use of its Energy Conservation Voluntary Performance Standards for Commercial and Multi-Family High-Rise Residential Buildings; Mandatory for New Federal Buildings; Interim Rule (referred to in this report as DOE-1993). A key requisite of the legislation requires DOE to develop commercial building energy standards that are cost effective. During the demonstration project, DOE specifically addressed this issue by assessing the impacts of the standards on (1) construction costs, (2) builders (and especially small builders) of multi-family, high-rise buildings, and (3) the ability of low-to moderate-income persons to purchase or rent units in such buildings. We explore the structure of a new family gauge symmetry U(3) and show its experimental signatures to search for. U(3) gauge bosons obviate an unwelcome deviation of the charged lepton mass formula with the running masses from that with the pole masses. The current structure of this model leads to flavor number violations via exchange of extra gauge bosons. We obtain bounds on the masses of the gauge bosons from rare kaon decay searches and muonium-antimuonium oscillation searches. We propose attractive signatures at LHC and lepton colliders and discuss feasibility of their discovery.

This article examines the interplay between globalising and localising forces occurring in a child welfare reform project in South Korea. Focusing especially on care and education services for children and families provided by the Korean 'Dream Start' programme, which provides comprehensive health, parental involvement and welfare services to low-income children and their families, I discuss its involvement in the process of 'glocalisation'. Drawing on Foucaultian ideas, I conducted a critical discourse analysis of the policy documents of Dream Start. The article concludes by discussing local resistance to the creation of a cosmopolitan child and the dual qualities of inclusion/exclusion inscribed in the Dream Start project.

Evidence from twin, adoption, and family studies suggests that there is strong aggregation of suicidal behaviors in some families. By comparison, the role of social modeling through peers has yet to be convincingly established. This paper uses data from four large studies (the WHO/EURO Multicentre Study on Suicidal Behaviour, the WHO/SUPRE-MISS, the CASE study, and the Queensland Suicide Register) to compare the effects of exposure to fatal and nonfatal suicidal behavior in family members and nonfamilial associates on the subsequent suicidal behavior of male and female respondents of different ages. Across all studies, we found that prior suicidal behaviors among respondents' social groups were more important predictors of suicidal behavior in the respondents themselves than previous research had indicated. The relative merits and difficulties of the primary and secondary origin hypotheses for the observed cosmic ray antiprotons, including the low energy measurement of Buffington, were examined. It is concluded that the cosmic ray antiproton data may be strong evidence for antimatter galaxies and baryon symmetric cosmology. The use of primary electron counting techniques as an alternative to the more usual parallel plate avalanche chamber that has been employed in soft x-ray scattering experiments is being investigated at the National Synchrotron Light Source. The theoretical aspects of primary electron counting and motivation behind building a primary electron counting detector are described, as well as characteristics and future improvements of the device constructed at the NSLS. The detector consists of a low electric field drift region and a low pressure multistep avalanche region which can be operated with two or three stages of electron multiplication. The device has worked well in extensive tests as a simple parallel plate avalanche chamber, providing energy resolutions of 58% and 43% at 277 and 500 eV, respectively. Magnetic properties of the Al1-xGaxFeO3 family of oxides crystallizing in a non-centrosymmetric space group have been investigated in detail along with structural aspects by employing X-ray and neutron diffraction, Moessbauer spectroscopy and other techniques. The study has revealed the occurrence of several interesting features related to unit cell parameters, site disorder and ionic size. Using first-principles density functional theory based calculations, we have attempted to understand how magnetic ordering and related properties in these oxides depend sensitively on disorder at the cation site. SNAP25 occurs on chromosome 20p12.2, which has been linked to schizophrenia in some samples, and recently linked to latent classes of psychotic illness in our sample. SNAP25 is crucial to synaptic functioning, may be involved in axonal growth and dendritic sprouting, and its expression may be decreased in schizophrenia. We genotyped 18 haplotype-tagging SNPs in SNAP25 in a sample of 270 Irish high-density families. Single marker and haplotype analyses were performed in FBAT and PDT. We adjusted for multiple testing by computing q values. Association was followed up in an independent sample of 657 cases and 411 controls. We tested for allelic effects on the clinical phenotype by using the method of sequential addition and 5 factor-derived scores of the OPCRIT. Nine of 18 SNPs had P values Irish family sample. Although we failed to replicate this in an independent sample, this gene should be further tested in other samples. PMID:19806613

Reactor Control Division (RCnD) has been one of the main designers of safety and safety related systems for power reactors. These systems have been built using in-house developed hardware. Since the present set of hardware was designed long ago, a need was felt to design a new family of hardware boards. A Working Group on Electronics Hardware Standardization (WG-EHS) was formed with an objective to develop a family of boards, which is general purpose enough to meet the requirements of the system designers/end users. RCnD undertook the responsibility of design, fabrication and testing of boards for embedded systems. VME and a proprietary I/O bus were selected as the two system buses. The boards have been designed based on present day technology and components. The intelligence of these boards has been implemented on FPGA/CPLD using VHDL. We genotyped 18 haplotype-tagging SNPs in SNAP25 in a sample of 270 Irish high-density families. Single marker and haplotype analyses were performed in FBAT and PDT. Data were collected on 154 adoptive families with gay/lesbian and straight adoptive parents (154 parent respondents and 210 adolescent respondents). This study was principally interested in factors affecting adolescent attachment including parent sexual orientation, adolescent and parent life satisfaction, and parent level of relationship satisfaction with their adopted child as well as other key parent, child and adoption characteristics. Evidence for triaxial nuclear shapes from families of unique-parity states in transitional odd-A nuclei around mass A=190 and A=130 is reviewed. The experimental data are analysed within the odd-A triaxial core model. Regular two-dimensional band patterns are found in experiment and are shown to be a consequence of broken axial symmetry. Recent theoretical developments are discussed including the question of how stable the triaxial shapes are.

We demonstrate the existence of shear-free cosmological models with rotation and expansion which support inflationary scenarios. The corresponding metrics belong to the family of spatially homogeneous models with the geometry of the closed universe (Bianchi type IX). We show that the global vorticity does not prevent inflation and can even accelerate it.

We describe exact cosmological solutions with rotation and expansion in the low-energy effective string theory. Families of regimes for discrete control systems are studied possessing a special quasi-controllability property that is similar to the Kalman controllability property. A new approach is proposed to estimate the amplitudes of transient regimes in quasi-controllable systems. Its essence is in obtaining of constructive a priori bounds for degree of overshooting in terms of the quasi-controllability measure. The results are applicable for analysis of transients, classical absolute stability problem and, especially, for stability problem for desynchronized (asynchronous, switching) systems.

Apoptosis, or programmed cell death (PCD), is a fundamental process that protects organismal integrity. In earlier work, we demonstrated that over-expression of either of two anti-apoptotic members of the BCL-2 family (BCL-2 or BCL-X L could elevate the frequency of radiation-induced mutations at the autosomal TK1 locus in human TK6 lymphoblasts that express wild-type TP53. Ectopic expression of BCL-X L also elevated the frequencies of double-strand break-induced gene conversion. The present work addresses an off-axis Cassegrain optical concentration system. The specific primary collector analyzed, a short focal length parabolic concentrator, is at the University of Florida`s Energy Park. A secondary hyperbolic reflective element was designed to redirect the solar radiation from the primary focal plane to an off-axis target on the polar axis of the primary concentrator. This ground level target will be required for planned experimental work. The analysis was performed using a numerical ray tracing procedure that incorporates both random and systematic errors due to slope and surface irregularities. The optimization process varied secondary element size, curvature, and offset angle, and yielded information required for optimum design. we report an ultrafast synchrotron x-ray phase contrast imaging study of the primary breakup mechanism of a coaxial air-assisted water jet. We demonstrate that there exist great similarities in the phenomenology of primary breakup with that of the secondary breakup. Especially, a membrane-mediated breakup mechanism dominates the breakup process for a wide range of air speeds. This finding reveals the intrinsic connections of these two breakup regimes and has deep implications on the unified theoretical approach in treating the breakup mechanism of high speed liquid jet.

Forty-three patients with basal and squamous cell carcinomata arising on the pinna were treated radically using a 10 MeV electron beam. The technique and dosage are described and discussed. Primary cancer control with retention of the pinna was achieved in 34 patients. Salvage pinnectomy was performed in four patients for recurrence and one patient for radiation necrosis. Two patients with large primary tumours failed to resolve and died of their disease. The advantages for the patient of the policy of primary radical electron beam therapy are discussed.

Twenty-two patients with vertebral primaries were registered in the Intergroup Ewing's Sarcoma Study between 1973 and 1977. The radiation doses to the primary tumors ranged between 3800 and 6200 rad. All patients received intensive combination chemotherapy. After a followup ranging between 14 and 62 months, 14 patients remained disease-free. All patients with primary tumor of the cervical and dorsal spine remained disease-free. Of eight patients with lesions in the distal spine, (sacrococcygeal region) six developed recurrence, in three a local recurrence was observed despite doses of 6000 rad or higher. Doses of 5000 rad or less (in addition to combination chemotherapy as used in the Intergroup Ewing's Study) appear adequate in controlling the primary tumors of the proximal segments of the spinal column.

Primary Ewing's sarcoma of the skull is a very rare entity. We report MRI findings in a case of Ewing's sarcoma of the greater wing of sphenoid in a 4-year-old patient. Magnetic resonance imaging showed markedly heterogenous signal intensity with areas of haemorrhage and necrosis.
