For diagnosis of loosening or infection after implantation of a single-side or double-side total hip endoprosthesis 59 patients being in pain were investigated by a combination of bone scan (Technetium-99m DPD) and leucocyte scan (Indium-111 labelled leucocytes). The correlation between clinical results and scintigraphic findings revealed: bone scintigraphy detects loosening of prostheses with 95% sensitivity (45/47); bone scintigraphy excludes an instable prosthesis with 89% specificity (3/27). 45% of the loosened prostheses revealed an infection (21/47): leucocyte scintigraphy could detect 17 correct positive. Three leucocyte scans of 53 prostheses without clinical signs of infection were false positive. Therefore, leucocyte scintigraphy had a sensitivity of 91% and a specificity of 94% for detecting infection. Objective: To develop software for assessment of dose to thyroid gland in emergencies resulting in the release of radioiodine. Methods: Based on procedures for thyroid monitoring and dose assessment introduced in IAEA publications (IAEA-TECDOC-1092 and EPR-MEDICAL-2005), a software was developed with Visual Basic 6.0 program to computerized the methods of thyroid dose estimation. Results: The intake of radioiodine and committed equivalent dose to thyroid could be estimated rapidly and accurately with the present software. Conclusion: The present software provide a practical and rapid tool for estimating thyroid doses received in emergencies resulting in the release of radioiodine by emergency workers and members of public, which is necessary for recommendation of further actions. The aim of this study was to evaluate the automated urine test strip analyzer Aution Max AX 4280 which uses strips able to measure 12 urinary parameters. For precision study, we considered glucose, protein, blood, leukocytes, nitrite, and pH and the four first were compared with usual methods: chemical measurement or microscopic examination. Reproducibility of the semi quantitative results was determined as the proportion of results falling into the same concentration range. Within-run reproducibility assessed using urine specimens were between 83% and 96%. Between day on quality controlled materials, it was higher (96%), and leukocytes were shared between two classes 250 and 500 cells/microL. Botulinum neurotoxin is produced by the bacterium, Clostridium botulinum. The neurotoxin inhibits acetylcholine at the neuromuscular junction, thus interfering with overall muscular contraction. Botulinum neurotoxin is commonly used for the following medical conditions: cervical dystonia, upper limb spasticity, blepharospasm, strabismus, and hyperhydrosis. However, the use of botulinum neurotoxin was recently approved for the prophylaxis of headaches in adults with chronic migraines. Real-time object detection is one of the core problems in computer vision. The cascade boosting framework proposed by Viola and Jones has become the standard for this problem. In this framework, the learning goal for each node is asymmetric, which is required to achieve a high detection rate and a moderate false positive rate. We develop new boosting algorithms to address this asymmetric learning problem. We show that our methods explicitly optimize asymmetric loss objectives in a totally corrective fashion. The methods are totally corrective in the sense that the coefficients of all selected weak classifiers are updated at each iteration. In contract, conventional boosting like AdaBoost is stage-wise in that only the current weak classifier's coefficient is updated. We developed prototype chemistry for nucleic acid hybridization on our bead-based diagnostics platform and we established an automatable bead handling protocol capable of 50 part-per-billion (ppb) sensitivity. We are working towards a platform capable of parallel, rapid (10 minute), raw sample testing for orthogonal (in this case nucleic acid and immunoassays) identification of biological (and other) threats in a single sensor microsystem. In this LDRD we developed the nucleic acid chemistry required for nucleic acid hybridization. Our goal is to place a non-cell associated RNA virus (Bovine Viral Diarrhea, BVD) on the beads for raw sample testing. Purpose: Hormonal stimulation can induce background enhancement (BE) in MR mammography (MRM). This fact has been assumed to decrease the accuracy of MRM. Consequently, this report investigates: 1. The prevalence of BE in postmenopausal vs. premenopausal women in correlation to hormonal cycle phase (CP). 2. The impact of hormonal status (HS) and BE on diagnostic accuracy. Materials and Methods: Consecutive patients over 22 months with complete HS information (week of CP or postmenopausal) were included in this prospective investigation. Aim of this study was to evaluate the sensitivity of virtual colonoscopy (CT colonography) in the identification of colorectal cancer and to define the limitations and the advantages of this imaging modality, as well as indications to the examination. It was examined prospectively 62 symptomatic patients aged 36 to 82 years (28 women and 34 men). All patients underwent both conventional and virtual colonoscopy on the same day; the conventional examination allowed exploration of the entire colon. Conventional colonoscopy identified 89 lesions 3-50 mm in diameter, namely 84 benign and 5 malignant lesions. No lesions were identified in 12 patients. CT colonography identified 52 of the 89 lesions, with 57.1% diagnostic accuracy. They were 11 false positives (82.5% positive predictive value and 52.2% specificity) and 37 false negatives (24.5% negative predictive value and 58.4% sensitivity). Twenty-two patients, all with surgically proven primary hyperparathyroidism, were studied by TI-201 thallous chloride and Tc-99m pertechnetate subtraction imaging. Fifteen parathyroid adenomata and one hyperplastic gland between 0.33 and 14.8 g were correctly localized in 16 patients. Two adenomata and seven hyperplastic or histologically normal parathyroids between 0.1 g and 1.4 g in seven patients were not localized. One patient had a correctly localized 13.0-g adenoma with a nonlocalized 0.3 g hyperplastic parathyroid gland and there were two false positive localizations. Sensitivity was 64% (glands), and 73% (patients). There was only fair correlation with parathormone (PTH) levels, but these were elevated in all but four of the patients with correctly localized parathyroids. A novel multilevel hierarchical Kohonen Net (K-Map) for an intrusion detection system is presented. Each level of the hierarchical map is modeled as a simple winner-take-all K-Map. One significant advantage of this multilevel hierarchical K-Map is its computational efficiency. Unlike other statistical anomaly detection methods such as nearest neighbor approach, K-means clustering or probabilistic analysis that employ distance computation in the feature space to identify the outliers, our approach does not involve costly point-to-point computation in organizing the data into clusters. Another advantage is the reduced network size. We use the classification capability of the K-Map on selected dimensions of data set in detecting anomalies. Randomly selected subsets that contain both attacks and normal records from the KDD Cup 1999 benchmark data are used to train the hierarchical net. We use a confidence measure to label the clusters. Purpose: To evaluate the diagnostic performance (colorectal lesions) of computed tomography (CT) colonography in 111 patients, a majority of whom were at high risk for colorectal neoplasia. Material and Methods: After bowel preparation, CT colonography was performed, immediately followed by conventional colonoscopy. The diagnostic performance of CT colonography was analyzed relative to lesion size, histological diagnosis, and diagnostic certainty. Results: The sensitivity of CT colonography increased with lesion size (P<0.001), and was 91% (21/23) for lesions > or = 10 mm. All 10 carcinomas and 86% (19/22) of adenomas > or = 5 mm were detected. Unconfirmed or false-positive CT findings were generally small and/or reported with low diagnostic certainty. Full text: The aim of this study was to determine the accuracy of CT colonography (Virtual colonoscopy; VC) for polyp detection in a community-based screening program of asymptomatic subjects at average-risk of colorectal cancer (CRC). Average-risk subjects of two quintiles (50-54 and 65-69 years) were randomly selected from the electoral roll and invited to participate in a VC-based CRC screening program. After bowel preparation, bowel insufflation with CO_2 and intravenous hyoscine butylbromide (unless contra-indicated), supine and prone scans were acquired with a single array GE CTi helical scanner. Scanning parameters were: collimation 3 or 5mm; pitch 1.5:1 or 2.0:1; reconstructions at 1-2mm; 70mAs (5mm collimation) or 120mAs (3mm collimation). Subjects in whom VC demonstrated a polyp were offered colonoscopy (CY) on the same day. The purpose was to evaluate low-dose CT colonography without cathartic cleansing in terms of image quality, polyp visualization and patient acceptance. Sixty-one patients scheduled for colonoscopy started a low-fiber diet, lactulose and amidotrizoic-acid for fecal tagging 2 days prior to the CT scan (standard dose, 5.8-8.2 mSv). The original raw data of 51 patients were modified and reconstructed at simulated 2.3 and 0.7 mSv levels. Two observers evaluated the standard dose scan regarding image quality and polyps. A third evaluated the presence of polyps at all three mSv levels in a blinded prospective way. All observers were blinded to the reference standard: colonoscopy. At three times patients were given questionnaires relating to their experiences and preference. Image quality was sufficient in all patients, but significantly lower in the cecum, sigmoid and rectum. A fast and simple flow-through optosensor was designed and characterized for the direct screening of four tetracycline (TCC) antibiotics (tetracycline, oxytetracycline, chlortetracycline and doxycycline) in water and bovine milk samples. The proposed optosensor provides rapid binary yes/no overall responses, being appropriate for the screening of this family of antibiotics above or below a pre-set concentration threshold. The experimental set-up is based on a flow-injection manifold coupled on-line to a phosphorescence detector. Aliquots of the samples are pretreated with Eu(III) to form room temperature phosphorescent metal chelates and injected in the flow manifold. Those chelates are then on-line retained on a conventional flow-cell (packed with polymeric Amberlite XAD-4 particles) which is placed inside the cell holder of the phosphorimeter. Ninety-five subjects underwent reduced-laxative (13 g senna/18 g magnesium citrate) CTC prior to same-day colonoscopy and were randomised to one of four tagging regimens using 20 ml 40%w/v barium sulphate: regimen A: four doses, B: three doses, C: three doses plus 220 ml 2.1% barium sulphate, or D: three doses plus 15 ml diatriazoate megluamine. Patient experience was assessed immediately after CTC and 1 week later. Two radiologists graded residual stool (1: none/scattered to 4: >50% circumference) and tagging efficacy for stool (1: untagged to 5: 100% tagged) and fluid (1: untagged, 2: layered, 3: tagged), noting the HU of tagged fluid. The emergence of bacterial antibiotic resistance is a growing problem, yet the variables that influence the rate of emergence of resistance are not well understood. In a microfluidic device designed to mimic naturally occurring bacterial niches, resistance of Escherichia coli to the antibiotic ciprofloxacin developed within 10 hours. Resistance emerged with as few as 100 bacteria in the initial inoculation. Whole-genome sequencing of the resistant organisms revealed that four functional single-nucleotide polymorphisms attained fixation. Knowledge about the rapid emergence of antibiotic resistance in the heterogeneous conditions within the mammalian body may be helpful in understanding the emergence of drug resistance during cancer chemotherapy. It was shown that certain types of telephone cards containing microchips have the potential to be used as individual radiation dosimeters in emergency situations to detect doses over 250 mGy by luminescence measurements. Boron Neutron Capture Therapy (BNCT) is a promising binary treatment modality for high-grade primary brain tumors (glioblastoma multiforme, GM) and other cancers. BNCT employs a boron-10 containing compound that preferentially accumulates in the cancer cells in the brain. Upon neutron capture by {sup 10}B energetic alpha particles and triton released at the absorption site kill the cancer cell. The goal of this work is to present a systematic Monte Carlo validation study on the clinical implementation of the enhanced dynamic wedges (EDWs) into the Pinnacle"3 (Philips Medical Systems, Fitchburg, WI) treatment planning system (TPS) and QA procedures for patient plan verification treated with EDWs. Modeling of EDW beams in the Pinnacle"3 TPS, which employs a collapsed-cone convolution superposition (CCCS) dose model, was based on a combination of measured open-beam data and the 'Golden Segmented Treatment Table' (GSTT) provided by Varian for each photon beam energy. In this paper, probabilistic safety assessment (PSA) of the station blackout (SBO) and 5th emergency diesel in Daya Bay NPP has been carried out, the calculation method of non-recover factors of power supplies is given, and sensitivity analysis on the connection duration of 5th emergency diesel has been executed. It is concluded that the core damage frequency (CDF) induced by SBO is relatively large, the addition of 5th emergency diesel is very helpful for the CDF reduction, and the connection duration of this diesel has great effect on the CDF reduction

Purpose : To obtain an emergency core cooling device in a FBR type reactor by utilizing heat pipes which are not actuated at usual operation condition but actuated reliably upon emergency. Constitution : A system for injecting heat medium into heat pipes is provided. By injecting the heat medium into the heat pipes upon emergency to actuate the heat pipes, the reactor core is cooled. During normal reactor operation, the inside of the heat pipes is evacuated from a vacuum pump and no heat medium is filled therein, whereby unnecessary heat loss during the normal operation can be prevented. To optimize the emergency care of patients with multiple injuries, a special emergency room with integrated radiological equipment has been developed. It consists of a linear array of a computed tomography system, a mobile C-arm with an integrated scattered radiation grid for conventional films and a C-arm fluorographic system. Aim of this study was to evaluate the sensitivity of virtual colonoscopy (CT colonography) in the identification of colorectal cancer and to define the limitations and the advantages of this imaging modality, as well as indications to the examination. It was examined prospectively 62 symptomatic patients aged 36 to 82 years (28 women and 34 men). All patients underwent both conventional and virtual colonoscopy on the same day; the conventional examination allowed exploration of the entire colon. Conventional colonoscopy identified 89 lesions 3-50 mm in diameter, namely 84 benign and 5 malignant lesions. No lesions were identified in 12 patients. CT colonography identified 52 of the 89 lesions, with 57.1% diagnostic accuracy. They were 11 false positives (82.5% positive predictive value and 52.2% specificity) and 37 false negatives (24.5% negative predictive value and 58.4% sensitivity). BACKGROUND: The urgent need for patient safety education for healthcare students has been recognised by many accreditation bodies, but to date there has been sporadic attention to undergraduate/graduate medical programmes. Medical students themselves have identified quality and safety of care as an important area of instruction; as future doctors and healthcare leaders, they must be prepared to practise safe healthcare. Medical education has yet to fully embrace patient safety concepts and principles into existing medical curricula. Sheltering, evacuation and distribution of stable iodine tablets are considered to be major early emergency actions aiming at diminishing the consequences after a release of radioactive materials from nuclear power plants into the air. Whether in real situations emergency managers will act accordingly is hard to predict. Uncertainties associated with these decisions are termed 'volitional' uncertainties. These uncertainties, however, cannot be assessed by expert judgements as they express the decision at stake in an emergency situation. Uncertainties on the times to implement countermeasures and on the times for the general population to respond to these measures can be assessed by experts, as they represent 'lack-of-knowledge' uncertainties. Weakly absorbing materials, such as biological, soft tissues, can be imaged by generating contrast due to the phase shift of X-rays. In the past decade, several methods for X-ray phase imaging were proposed and demonstrated. The performance of X-ray phase imaging is attractive in the field of medical imaging technology, and its development for practical use is expected. Many methods, however, have been developed under the assumption of the use of synchrotron radiation, which is an obstacle to practical use. The method based on Talbot (-Lau) interferometry enables us to use a compact X-ray source, and its development is expected as a breakthrough for medical applications. (author)

Surveys for interior wetland birds at mine-associated emergent wetlands on coal surface mines in southern Illinois detected one state threatened and two state endangered species. Breeding by least bittern (Ixobrychus exilis) and common moorhen (Gallinula chloropus) was confirmed. Regional assessment of potential wetland bird habitat south of Illinois Interstate 64 identified a total of 8,109 ha of emergent stable water wetlands; 10% were associated with mining. Mine-associated wetlands with persistent hydrology and large expanses of emergent vegetation provide habitat that could potentially compensate for loss of natural wetlands in Illinois.

An introduction is presented on the radiation hazards connected with biomedical radiography and nuclear medicine. The frequency of radiodiagnostic efforts was rather high in the Netherlands. This was reduced considerably by abolishing the thorax screening of the population. About diagnostic nuclear medicine less can be said because far fewer numerical data are available. When evaluating the risks of oncogenesis and cancer mortality following exposure to the radiations of the atomic bombs (A-bombs), the medical X-ray doses received by the A-bomb survivors must also be estimated and considered. Using a human phantom, dosimetry was performed to estimate the X-ray doses received by A-bomb survivors during medical examinations at the Radiation Effects Foundation (RERF) as part of the long-term follow-up on the Adult Health Study (AHS). These examinations have been estimated to represent nearly 45% of the survivors' cumulative medical irradiation dose. Doses to the salivary glands, thyroid gland, lung, breast, stomach and colon were measured using thermoluminescent dosemeters. The results, which are reported here, will aid in estimating organ doses received by individual AHS participants. (Author).

Medical Science Liaison (MSL) teams are a well-recognized function within the pharmaceutical and medical products industries. While in most organizations, MSLs have focused on the development of relationships with individual Key Opinion Leaders, progressive companies have realized the value that MSLs can also bring to Key Account Management (KAM). This article discusses the five steps that should be followed in order to successfully integrate the support of Key Accounts into an MSL function. These include understanding the business approach to KAM, identifying what MSL resources are available, developing a comprehensive strategic plan, implementing the plan, and evaluating and making any necessary adjustments. During the normal operation of a Nuclear Power Plant (NPP), radioactive releases into the atmosphere will be in small quantities. During major accidental situations, though the probability is extremely small, there may be significant release of radioactivity to the environment through the stack or at ground level. To study the external radiation exposure, if any, to the members of public due to releases during the normal operation of a nuclear power plant (NPP) and also to meet the requirement of emergency preparedness for the NPP site, continuous recording and analysis of environmental dose rate data is essential. This paper presents analysis of the gamma dose rates recorded by the Environmental Dose Logging Systems (EDLS) installed around the site during the last six years in the Emergency Planning Zone (EPZ) of Tarapur Atomic Power Station (TAPS). The Lawrence Livermore National Laboratory (LLNL) has the necessary equipment and trained personnel to respond to a large number of hazardous material spills and fires or other emergencies resulting from these spills including injured personnel. This response capability is further expanded by the agreements that LLNL has with a number of outside response agencies. The Hazards Control Department at LLNL functions as the central point for coordinating the response of the equipment and personnel. Emergencies involving hazardous waste are also coordinated through the Hazards Control Department, but the equipment and personnel in the Toxic Waste Control Group would be activated for large volume waste pumpouts. Descriptions of response equipment, hazardous waste locations communication systems, and procedures for personnel involved in the emergency are provided.
