It is reported on examination-technical possibilities with craniocerebral Computer Tomography in the peri- and postnatal period. Some typical tomographic images from a 17 1/2 months period in our own patient material of 327 children are demonstrated. The special advantages of this new technical-extensive method are: exact diagnoses, observation possibility of the longitudinal section, and the absolute harmlessness to the child.

Intercrosses of heterozygous pro-opiomelanocortin (POMC) mice result in homozygous null progeny at lower frequencies than expected. Genotyping offspring at pre-, peri-, and postnatal stages revealed that over half of homozygous null mutants die in the early postnatal stages. To investigate the reasons for this early postnatal lethality, we analyzed in detail different parameters in the initial hours after birth. POMC null mutants born to heterozygous dams presented at birth with corticosterone levels no different from wildtype littermates, were euglycemic, and had normal liver glycogen stores. However, already 30 min after birth corticosterone levels dropped by 80% and were undetectable thereafter, while corticosterone levels in wildtype animals increased during postnatal hours. Ingestion of HTO during oocyte maturation and continued during pre-implantation time was found to depress decidual response. At birth these offspring also showed a decrease in brain cell number. When HTO was given during pregnancy only, the offspring at birth showed a similar deficit in brain cell number. Even so, we could not demonstrate a gross deficit in oocyte maturation when HTO was ingested during sexual maturity only; however, when further continued during pregnancy, the measured newborn parameters were most severely affected. In this group, cell-multiplication in the cerebrum was severely decreased, protein synthesis was decreased, and somatic growth was also highly significantly decreased. These experiments indicate that even a short-term exposure to HTO during early pregnancy (pre-implantation) alters normal development to such an extent that it is still observable at birth. Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis is a rare entity, sometimes referred to as Herlyn-Werner-Wunderlich syndrome (HWW). It usually presents after menarche with progressive pelvic pain, sometimes with regular menses, and a palpable mass due to hemihaematocolpos. The diagnosis is generally made only if the suspicion of this genitourinary syndrome is raised. To highlight the imaging diagnostic clues in this rare condition. We report on 11 adolescents with this condition. Sonography mostly allowed the correct diagnosis by showing uterovaginal duplication, haematocolpos or haematometrocolpos, and the absence of the ipsilateral kidney. The possible long-term effects of chronic radiation exposure have been studied. Rats were given tritiated drinking water (3 #mu#Ci/ml) before and during pregnancy and thereafter continuously through five generations. The brains were studied at 30 and 120 days postnatally. Significant decreases in weight and DNA and protein contents of specific parts of the brain were found, the most pronounced decreases being in the diencephalon. The decrease in protein content were more pronounced than the decreases in DNA content; the decreases were also more pronounced at 30 days than at 120 days, and more pronounced in the fith generation than in previous generations.

We consider a serial supply chain consisting of a raw material supplier, a manufacturer, a distribution centre and a retailer in the presence of time-varying delivery between manufacturer facility and the retailer warehouse. Delivery time functions are developed based on practical data analysis and the cost models for both linear and non-linear delivery time functions are derived. Analytic solution for system with linear delivery times is derived and a search algorithm for system with non-linear delivery times is established. Finally, sensitivity analysis is made to help decision makers achieve a lower total cost in practice.

Therefore, this quarterly progress report summarizes the period's activities in Engineering, Procurement and Civil Construction. Engineering is 95% complete, but revisions and changes are still being incorporated particularly in the Instrument and Electrical areas. Procurement has only valves 4 in. and above yet to order to complete all major items purchased. The scheduled material curve was based on the original estimate of the job. The projected curve reflects the previous month's estimate. Construction of civil work is proceeding according to schedule in the field. The initial bid packages for the Mechanical General Contractor portion of the work have been reviewed and evaluated. Order placement is scheduled for the end of July. A low nephron endowment may be associated with hypertension. Nephrogenesis is the process that leads to the formation of nephrons until week 36 of gestation in humans and may be inhibited by many factors like intrauterine growth restriction and premature birth. To study the consequences of a low glomerular number, animal models have been developed. We describe a model of postnatal food restriction in the rat in which litter size is increased to 20 pups, which leads to growth restriction. In the rat, active nephrogenesis continues until postnatal day 8, which coincides with the growth restriction in our model. Design-based stereological methods were used to estimate glomerular number and volume. Our results show an approximately 25% lower glomerular number in rats after postnatal food restriction (30,800 glomeruli/kidney) compared with control rats (39,600 glomeruli/kidney, P < 0.001). Pregnant females and newborn rats were exposed to a gamma irradiation of 150 rads. The stage of gestation at the time of irradiation varied from 14 to 21 days. The newborn rats were irradiated at 0, 1 and 2 days of age. The effect of irradiation of foetus and newborn rats depends on the age of the animal at the time of irradiation. This effect was specially important at the beginning of the foetal life. Neonatal mortality, growth of body weight and adult brain development were investigated. A modification of germ cell radiosensitivity during the period studied, was emphasized.

Male meadow voles maintained in a long photoperiod (LP) from birth have heavier brains than do females, but in short photoperiods (SP) this sex dimorphism is absent. Testosterone propionate (TP) administration on the second day of postnatal life produced significant increases in brain weight of LP but not SP females at 35 days of age. Short daylengths reduce the responsiveness of the meadow vole nervous system to the masculinizing effects of perinatal testosterone and may, in part, mediate the seasonally reduced sex difference in brain weight. Objective: To find out safety and efficiency of Misoprostol in cervical ripening and induction of labour to achieve vaginal delivery. Results: From Misoprostol insertion to delivery time was 4-24 hours. Vaginal delivery was achieved in 80.2%, which included spontaneous, forceps and vacuum extraction. Caesarean section rate was 19.7%. Indications for C. Section included Misoprostol unresponsiveness 11% and fetal distress in 8.6%. Oxytocin augmentation was required in 32% of cases. Term babies were 58%. Intrauterine death and neonatal deaths were 9.8% and 8.6% respectively. Hyper stimulation and postpartum haemorrhage was seen in 2.4% and 3.7% of patients respectively. Conclusion: intravaginal Misoprostol is well tolerated and is very effective for the induction of labour in eclampsia. It helps vaginal delivery in toxemic patients, reduces maternal morbidity, mortality and hospital stay. (author)

Still, there were three major changes that initially caused serious problems for Purple users. The first was making 64-bit builds of executables the default instead of 32-bit. The second was requiring all executables to use large page memory. The third was the phase-out of the popular, but now defunct, third-party C++ compiler KCC, which required the migration of many codes to IBM's xlC C++ compiler. On Purple, the default build environment changed from 32-bit builds to 64-bit builds in order to enable executables to use the 4GB per processor (32GB per node) memory available, and in order for the MPI library to do collective optimizations that required the larger 64-bit address space. A fluidic pump comprises a two-diode pump having a first displacement vessel, a third diode connected to receive output from the two-diode pump through a delivery line and discharge to an outlet, and a second displacement vessel connected to the delivery line. There is a feed tank at a greater height than the two-diode pump, and a drive unit for alternately pressurising and venting the first and second displacement vessels. This paper investigates regional and age-related changes in metabolite ratios using image-localized H-1 MR spectroscopy in children. Twenty-seven children referred for MR imaging of the brain underwent localized H-1 MR spectroscopy with a Siemens 1.5-T whole-body imager. Localization to 2 x 2 x 2-cm volumes of interest was achieved with a stimulated-echo or spin-echo technique. Repetition times of 1,600 and 5,000 msec were used, with echo times varying between 68 and 270 msec. Total combined imaging and spectroscopy times were 90-120 minutes.

Flexible delivery reliability (FLP), which is in focus in this report, means that the customer shall be able to choose from a number of different grid tariffs of different qualities. This way he shows his actual willingness to pay for quality. It is an important difference compared with the KILE system, which is based on a surveyed willingness to pay. As it is planned to be implemented, FLP assumes that the customer must choose one of a number of different subscriptions and that he makes his choice based on his own preferences and costs what suits him best. The various subscribers entail different degrees of delivery reliability. For the grid companies, customers and regulatory authorities the FLP represents a challenge. It is assumed that the customers have a conscious attitude or preference with respect to delivery quality. That is not always the case today.

We present two cases of fetal neck masses that were initially diagnosed by ultrasound and further evaluated with prenatal MRI. MRI findings aided in further delineating the neck masses, increasing confidence in the final diagnosis (cervical teratoma and cystic hygroma). With the fetal airway typically filled with fluid that is of high signal on T2-weighted sequences, MRI images in three planes could identify whether the fetal larynx and trachea were partially or completely compressed by the neck tumor. This information was particularly useful in determining if a controlled delivery such as ex utero intrapartum treatment (EXIT) was necessary and aided the surgeons in planning their approach to establishing airway control in the delivery room. A variable gene delivery system has been developed based on conjugating chitosan to biotin through a functionalized poly(ethylene glycol) (PEG) spacer, which can be used to further bind different molecules on the outer layer of a polymer/DNA complex by streptavidin (SA)-biotin linkage. In this study, TAT-conjugated SA was used as the model molecule to prove the conjugation function of the prepared complex. In addition, low-molecular-weight poly(ethyleneimine) (PEI) was added into the polymer/DNA complex to increase the transfection efficiency. The results of the luciferase assay show that the transfection efficiency of the prepared complex was significantly correlated with the amount of PEI and was further enhanced when TAT was conjugated to the complex by SA-biotin linkage. A novel drug delivery system for the treatment of brain tumors was formulated by methotrexate (MTX)-loaded polymeric nanoparticles (NPs) based on Glycol chitosan (GCS) and Dextran sulfate (DS). The physicochemical properties of resulting particles were investigated, evidencing the contribution of these nanoparticles for brain targeting. In vitro release of MTX was also evaluated. This study addresses the modeling of transdermal diffusion of drugs, to better understand the permeation of molecules through the skin, and especially the stratum corneum, which forms the main permeation barrier of the skin. In transdermal delivery of systemic drugs, the drugs diffuse from a patch placed on the skin through the epidermis to the underlying blood vessels. The epidermis is the outermost layer of the skin and can be further divided into the stratum corneum (SC) and the viable epidermis layers. The SC consists of keratinous cells (corneocytes) embedded in the lipid multi-bilayers of the intercellular space. It is widely accepted that the barrier properties of the skin mostly arises from the ordered structure of the lipid bilayers. Dose reconstruction is a process that re-creates the treatment-time dose deposited in a patient provided there is knowledge of the delivered energy fluence and the patient's anatomy at the time of treatment. A method for reconstructing dose is presented. The process starts with delivery verification, in which the incident energy fluence from a treatment is computed using the exit detector signal and a transfer matrix to convert the detector signal to energy fluence. With the verified energy fluence and a CT image of the patient in the treatment position, the treatment-time dose distribution is computed using any model-based algorithm such as convolution/superposition or Monte Carlo. The accuracy of dose reconstruction and the ability of the process to reveal delivery errors are presented. The goals of this paper are to describe new strategies being pursued at several institutions for the brain-specific delivery of radionuclides that can be used to evaluate regional cerebral perfusion by single photon imaging techniques. A comprehensive review of the literature is beyond the scope of these proceedings and our goal is to, therefore, present a description of several examples of interesting new strategies that have recently been reported. In addition, the authors also describe a new approach being pursued at their institution for the brain-specific delivery of radioiodinated iodophenylaklyl-substituted dihydronicotiamide systems which shows good brain uptake and retention in preliminary studies in rats. Following transport into the brain these agents appear to undergo facile intracerebral oxidation to the quaternized analogues which do not cross the intact blood-brain-barrier and are effectively trapped in the brain.

Perinatal exposure to cadmium (Cd) or methylmercury (MeHg) results in impaired neurodevelopment. Thyroid hormone is essential for normal brain development. However, the issue whether Cd or MeHg, especially at low doses, interrupts thyroid hormone action remains to be investigated. In the present study, effects of perinatal exposure to low levels of Cd or MeHg on thyroid hormone metabolism were examined using metallothionein I and II (MT-I/II) null or wild-type neonatal mice. Dams were exposed to 10 mg/L water of Cd or 5 mg/kg chow of MeHg from gestational day 0 to post-natal day 10 (PND 10). Sera, livers and brains were collected from neonates on PND 10. Iodothyronine deiodinase activities and serum thyroxine (T4) concentrations were measured. MeHg exposure failed to induce changes in serum T4 levels and liver type 1 deiodinase (D1) and brain type 2 deiodinase (D2) activities regardless of the MT genotype. The late effects of "2"3"9Pu have been studied after its administration to perinatal, juvenile and adult rats. Adults and weanlings were injected intravenously, newborns intracardially and 19-day foetuses by intravenous injection of the dam. Dose levels were selected to deliver radiation doses of approximately 7, 23, or 70 rads to the femur of all ages during the first 10 days post injection; in general these values were achieved as shown by tissue analysis. Dose rates subsequently diverged, resulting in widely varying cumulative radiation doses for the four age groups. Survival times of rats exposed post-natally were progressively decreased by increasing dose. No decrement in survival was found for those exposed prenatally. Bone tumour incidence was increased by exposure to Pu in all age groups. This model allows for multifaceted programs that target individuals, groups, organizations, and the entire community to alter their cardiovascular risk through managing blood pressure, lowering blood cholesterol, quitting smoking, increasing fitness, and maintaining desirable weight levels. The use of the cervicograph with the incorporated action and alert lines in labour management in developing country obstetrics is now well established and patients in labour in Port Moresby have been thus managed since 1973. In this study the cerviograph was introduced in hospitals and health centres in Enga Province and the results analysed. Despite intensive tuition in its use, a large number of cervicographs were incorrectly constructed, especially in health centres, and its introduction made little difference to operative delivery or perinatal mortality rates. The proportion of patients crossing the action line was much higher than in Post Moresby but this difference was probably also largely due to the inaccuracy of many cervicographs. This study involved 11 cases of full term vertex delivery in which FHR was recorded by fetal direct ECG during the second stage labor. All infants weighed 2,500 g or more. FHR monitoring was evaluated by Hon's classification. In this report, magnetite nanocrystal clusters with high surface areas were fabricated through a solvothermal process by introducing ammonium acetate as a porogen and trisodium citrate as a surface modification agent. The porosity, which was controlled by the reactant concentration, has been investigated in detail. The surface area of the nanocrystal clusters was as high as 141?m(2) g(-1) . Childhood anxiety is impairing and associated with later emotional disorders. Studying risk factors for child anxiety may allow earlier identification of at-risk children for prevention efforts. This study applied an ecological risk model to address how early childhood anxiety symptoms, child temperament, maternal anxiety and depression symptoms, violence exposure, and sociodemographic risk factors predict school-aged anxiety symptoms. This longitudinal, prospective study was conducted in a representative birth cohort (n = 1109). Six patients with diskitis following lumbar disk surgery were investigated by Computed Tomography (CT). Four patients underwent CT in a rather early phase of the disease (5-37 days after the onset of the symptoms): a posterior paravertebral fluid collection was always recognized, while the plain film was normal. Later on (2-4 months), the classical signs of diskitis were evident, with both CT and the plain film. The collection was always present at CT. A reintervention, performed on 3 patients, revealed the purulent content of the collections. It seems reasonable to suggest a relationship between the collection and the subsequent diskitis; the collection is probably an early sign of the disease. Its detection is of grat value, because it could allow an immediate and adequate therapy. The authors stress the usefulness of performing CT in an early phase on this kind of symptomatic patients.

Necrotizing enterocolitis (NEC) is the most common severe gastrointestinal emergency that affects premature newborns. This disease often has a rapid onset with few, if any, antecedent signs that can be used to reliably predict its occurrence. Its rapid onset and progression to death, as well as its severe morbidity when the infant survives, begs for early diagnostic tools that may be used in determining those infants who would be at greatest risk for development of the disease and for whom early preventative measures could be targeted. Although studies have suggested efficacy of several techniques such as breath hydrogen, inflammatory mediators in blood, urine or stool, and genetic markers, these all have drawbacks limiting their use. The application of newly developed "omic" approaches may provide biomarkers for early diagnosis and targeted prevention of this disease. PMID:19190533

Early effects of boron neutron capture therapy (BNCT) on malignant glioma are characterized by reduction of the enhancement area and regression of the peritumoral edema radiologically. The aim of this study was to investigate the early histological changes of tumors and inflammatory cells after BNCT in the rat brain. Rats were treated with BNCT using boronophenylalanine (BPA) 7 days after implantation of C6 glioma cells. The tumors were assessed with magnetic resonance imaging and histopathological examination at 4 days after BNCT. The mean tumor volumes were 39#+-#2 mm"3 in the BNCT group and 134#+-#18 mm"3 in the control group. In the BNCT group, tumor cells showed a less pleomorphic appearance with atypical nuclei and mitotic figures. The Ki-67 labeling index was 6.5%#+-#4.7% in the BNCT and 35%#+-#3.8% in the control group. The reactions of the inflammatory cells were examined with ED-1 as macrophage marker and OX42 as microglia marker. ...

The nature of the first generation of stars in the Universe remains largely unknown. Observations imply the existence of massive primordial stars early in the history of the universe, and the standard theory for the growth of cosmic structure predicts that structures grow hierarchically through gravitational instability. We have developed an ab initio computer simulation of the formation of primordial stars that follows the relevant atomic and molecular processes in a primordial gas in an expanding universe. The results show that primeval density fluctuations left over from the Big Bang can drive the formation of a tiny protostar with a mass of just one percent that of the sun. The protostar is a seed for the subsequent formation of a massive primordial star.

This article reviews antisocial personality disorder, psychopathy, and violence and develops a three factor model of personality traits. Then a discussion of related personality disorders precedes the development of a categorical two factor model of impulsive versus remorseless violence. A paradigm of proactive, medical, and school based early intervention and prevention is advocated as a useful addition to the reactive detention of criminal justice. Integration of psychological tests, neuroimaging, and genomic data in early childhood and school based intervention strategies to prevent the development of conduct disorder and attenuate criminal propensity inform this approach. The fact that South Africa had no commercially significant deposits of crude oil necessitated the early development of production of gasoline from coal which was present in abundance in low to medium grade in thick seams, the development of the process since the early 1950s is discussed. Both the Fischer Tropsch and Lurgi processes are used, and three large plants named Sasol One, Two, and Three have been completed. A flow diagram for the Lurgi process as used in these plants is included. Typical products and their properties resulting from the Sasol-type Synthol operation are presented in tabular form. Marketing considerations and economics of production of the Sasol products are discussed briefly. (BLM)

A high voltage self-blowing circuit breaker with closing resistors is equipped with an auxiliary contact inserted in the trip control circuit of an electrical circuit breaker. The auxiliary contact is actuated by the mechanical control rod of the circuit breaker in such a way as to prevent any operator closing error. A high-speed mechanism is inserted in the link system connecting the control rod and the auxiliary contact, with dead travel to delay closing of the auxiliary contact when a closing operation takes place and to prevent a tripping order from being transmitted too early. Such an early transmission would be liable to cause a flashover on the inserter contacts and damage to the circuit breaker.

We present our initial experience with a new biodegradable (BD) esophageal stent in two patients, one for a therapy-resistant benign esophageal stricture, and the other as a temporary measure during curative radiotherapy for oesophageal carcinoma. The BD stents need to be loaded into a conventional pull-back delivery system but are then placed in a standard fashion. Pre-dilatation should be avoided to reduce the risk of migration, however if migration occurs the stents can be left to dissolve in the stomach. The stents are radiolucent but easily identified on CT with minimal artefact and thus might even aid with radiotherapy planning. BD stents offer an exciting new strategy for therapy-resistant benign strictures as well as a supportive measure for oesophageal cancer undergoing non-surgical treatment. (orig.)
