A large proportion of patients may develop chronic pain following cancer treatments such as surgery, radiotherapy, or chemotherapy. These patients can experience significant levels of physical and psychological morbidity. Our aim was to investigate a cognitive-behavioral pain management program (PMP) for cancer patients with chronic treatment-related pain. Thirteen patients (1 man, 12 women; mean age 52 yrs) completed the study, 9 of whom had a history of breast cancer and had received extensive medical treatment, including surgery. A combination of physical and psychological techniques were adapted from previous work in chronic benign pain and implemented by two therapists. Locomotive engineers (train drivers) on irregular work schedules reported a general coffee consumption rate higher than that reported by a comparison sample of permanent shift factory workers. The present study examines the impact of this consumption on workday and non-workday sleep behaviour and mood ratings. Twenty-seven engineers and their spouses each completed daily logs for 30 consecutive days. Daily logs were then sorted into workday and non-workday categories. Workday sleep length was significantly shorter than non-workday sleep length for both engineers and spouses. For the engineers only, coffee consumption on workdays was higher than on non-workdays. This increased coffee consumption was correlated with longer sleep latency, increased negative mood, and decreased positive mood on both work and non-workdays. This was not true for spouses. PURPOSE.- The study aims to clinically validate the defining characteristics (DCs) of the nursing diagnosis (ND) of Activity Intolerance for patients with ischemic heart disease and refractory angina. METHODS.- Cross-sectional study was used, involving 22 patients with ND of Activity Intolerance. The Fehring method was used to validate the ND. FINDINGS.- Most DCs presented reliability indexes between 0.5 and 0.79. Three DCs presented reliability indexes - 0.8. CONCLUSION.- All DCs were validated, and electrocardiographic changes indicating ischemia, verbal report of fatigue, and abnormal rate response to activity were considered as DC major. The aims of the study were to develop and test the psychometric properties of the Thai Family Health Routines (TFHR) scale, a 70-item self-report questionnaire used to measure the health of Thai families through their routine behaviors in daily life. Development of the TFHR was based on the structural domains of Denham's Family Health Model. The TFHR scale was initially composed of 85 items and tested on 1,040 families living in the central region of Thailand. The confirmatory factor analysis, with an acceptable factor structure model, yielded 70 items aligned with six factors: self-care, safety and prevention, mental health behavior, family care, family caregiving, and illness care routines. The preliminary psychometric properties demonstrated that the TFHR scale had satisfactory internal consistency, criterion validity, and construct validity. Therapeutic effect of p-aminobenzoic acid-N-xyloside Na (K-247) were studied. Eleven patients with a variety of solid tumors were treated with K-247 alone. K-247 was given orally 800 mg daily for 4 weeks. As for side effect of the drug, only mild gastritis was observed in a few patients. Partial response (over 25% reduction of tumor size) with a median duration of two months was observed in 3 patients. These cases were metastatic tumor of lung from the carcinoma of thyroid, metastatic tumors of lung from the carcinoma of kidney, and mediastinal tumor. In eight patients the response was classified as no change and in one patient there was progressive disease. Thus K-247 has some therapeutic activity in patients with solid tumor. Combination therapy of irradiation and administration of K-247 were also studied. SRS provides good tumor control for small brain metastases from various primary cancers, with minimal untoward effects on surrounding normal brain. Purpose: Radiotherapy (RT) has been the predominant local treatment for Ewing's sarcoma of bone at the University of Florida. Twice-daily hyperfractionated RT was initiated in 1982 to improve local control and functional outcome. This retrospective review compares the results of once-daily vs. twice-daily RT in patients with primary Ewing's sarcoma of an extremity, with emphasis on functional outcome. Methods and Materials: Between June 1971 and January 1990, 37 patients were treated at the University of Florida for nonmetastatic Ewing's sarcoma of bone with a primary lesion in an extremity. Three patients underwent amputation. Of 34 patients treated with RT, 31 had RT alone and 3 had a combination of RT and local excision. A standard glazed flat plate collector and an evacuated tube collector are installed in parallel and tested at the same working conditions; the evacuated collector is a direct flow through type with external compound parabolic concentrator (CPC) reflectors. Efficiency in steady-state and quasi-dynamic conditions is measured following the standard and it is compared with the input/output curves measured for the whole day. Conventional subharmonic bunchers are currently used with rf linacs to generate pulse trains, but the resulting dilution of the transverse phase space and lower beam brightness are unacceptable for high-performance FELs. Recent developments suggest that photoemitters of high quantum efficiency combined with rapid acceleration can produce pulse trains of higher brightness than has been achieved before.

In the treatment of prostatic cancer using a newly proposed intermittent multiportal therapy (DIMT), acute reactions, change in tumor marker levels, and complications more than 6 months after treatment were evaluated. From June 1989 to September 1990, 26 patients with prostatic cancer (stage A2, 3 patients; B, 9; C, 2; D, 10; recurrence, 2) were treated. Fifteen patients were followed up for more than one year, with a mean period of 11.6 months. The treatment schedule is 52.5 Gy in 16 fractions in 4 weeks for radical treatment, and 30.0 Gy in 8 fractions in 2 weeks for palliative treatment. The 360 degree rotation about the patient was divided into 16 fractions, and 2 opposing fractions were used in one day as parallel opposed portals to treat the target volume. The fractions were serially treated one per day, so that 8 treatment days produced a total dose of distribution similar to that for conventional conformational therapy. Purpose: To assess the prognostic value of biologic (p53, Ki-67) and clinical factors in squamous cell carcinoma of the oropharynx after radical surgery and postoperative radiotherapy (RT). Methods and Materials: Between 1985 and 1995, a total of 102 patients with 104 tumor sites were entered onto the study. Fifty-five primary tumors (53%) involved the tonsils, 26 (25%) the soft palate, and 23 (22%) the base of the tongue. Median age was 53 years (range 36-80 years). The clinical T- and N-categories (UICC 1997) were: T1 (30), T2 (47), T3 (22), T4 (5), N0 (33), N1 (28), N2 (42), and N3 (1). Histologically-clear margins were achieved in all patients by initial surgery. Postoperative RT to the primary and regional lymphatics was given, to a total of 60 Gy in 6 weeks, and single daily fractions of 2 Gy. Exercise training associated with robust conditioning can be useful for the study of molecular mechanisms underlying exercise-induced cardiac hypertrophy. A swimming apparatus is described to control training regimens in terms of duration, load, and frequency of exercise. Mice were submitted to 60- vs 90-min session/day, once vs twice a day, with 2 or 4% of the weight of the mouse or no workload attached to the tail, for 4 vs 6 weeks of exercise training. Blood pressure was unchanged in all groups while resting heart rate decreased in the trained groups (8-18%). Skeletal muscle citrate synthase activity, measured spectrophotometrically, increased (45-58%) only as a result of duration and frequency-controlled exercise training, indicating that endurance conditioning was obtained. Ba, Cd, Cr, Cu, Fe, Ni, Pb and Zn were determined in birch leaves (Folium Betulae), dandelion roots (Radix Taraxacae), hawthorn blossom (Inflorescentia Crataegi) and their infusions by graphite furnace atomic absorption spectrometry (GFAAS) after microwave digestion of plant samples. Infusions were made from herbs according to prescription for patients, provided by the producer of medicine on the package. The results obtained were compared with daily requirements for each element. Results show high content of cadmium in the medicinal plants analyzed. The highest level in infusions was observed for Ni and Zn (over 90% of the total element concentration for Ni and in most cases over 50% for Zn), and the lowest for Cd and Pb. The calculated daily intake of majority of the analyzed elements was very low (under 1% of daily requirements)

The effect of 10 wk endurance swim training on 3-O-methylglucose (3-MG) uptake (at 40 mM 3-MG) in skeletal muscle was studied in the perfused rat hindquarter. Training resulted in an increase of approximately 33% for maximum insulin-stimulated 3-MG transport in fast-twitch red fibers and an increase of approximately 33% for contraction-stimulated transport in slow-twitch red fibers compared with nonexercised sedentary muscle. A fully additive effect of insulin and contractions was observed both in trained and untrained muscle. Compared with transport in control rats subjected to an almost exhaustive single exercise session the day before experiment both maximum insulin- and contraction-stimulated transport rates were increased in all muscle types in trained rats. Ensemble weather models can be used to produce medium range forecasts of weather events and to predict the likely available kilowatt-hours (kWhrs) of solar energy. As such, solar sites can maximize their harvest and use of solar energy. This presentation highlighted several different types of renewable energy forecasts obtained from weather models, including forecasts of expected kWhrs from solar panels and wind turbines, daily forecasts of expected solar heated water volumes and forecasts of water collection potential from impending precipitation events. Non-linear autoregressive Markov regime-switching models are intuitive. Time-series approaches for the modelling of electricity spot prices are frequently proposed. In this paper, such models are compared with an ordinary linear autoregressive model with regard to their forecast performances. The study is carried out using German daily spot-prices from the European Energy Exchange in Leipzig. Four non-linear models are used for the forecast study. This paper deals with an artificial neural network (ANN) based adaptive conventional power system stabilizer (PSS). The ANN comprises an input layer, a hidden layer and an output layer. The input vector to the ANN comprises real power (P) and reactive power (Q), while the output vector comprises optimum PSS parameters. A systematic approach for generating training set covering wide range of operating conditions, is presented. The ANN has been trained using back-propagation training algorithm. Investigations reveal that the dynamic performance of ANN based adaptive conventional PSS is quite insensitive to wide variations in loading conditions.

The effect of exercise training prior to acute myocardial infarction (AMI) on left ventricular (LV) remodeling is poorly understood. This study investigated the protective effect of 3 weeks of swimming exercise training prior to AMI on cardiac morphology and function. Male Sprague-Dawley rats (n = 35) were randomly assigned to 3 groups: swimming training (n = 14, 90 min, 5 days/wk, 3 wk), sedentary (n =14), and controls (n = 7, no exercise, no MI). At the end of the training/sedentary period, rats were subjected to AMI (ExMI and SedMI) induced by surgical ligation of the left coronary artery. Thereafter, the rats remained sedentary for a 4-wk recovery period. Two third-grade Taiwanese students at risk for emotional and behavioral disorders participated in a pull-out, small-group social skills training program developed to promote their skill acquisition and maintenance. Using a multiple baseline across skills design, the authors demonstrated that both participants made marked performance improvement in all three targeted social skills of on-task, appropriate conflict resolution, and cooperation during the intervention condition. Maintenance and follow-up data showed that both participants sustained their use of taught social skills in the training setting and their third-grade classroom, respectively. Social validity measures, including peer sociometric ratings and consumer satisfaction questionnaires and interviews, supported the positive effects of the social skills training program for both participants. In addition, relative interfractional tumour and lymph node motion is determined for each RT fraction. Material and methods. From November 2009 to March 2010, 20 consecutive lung cancer patients (14 NSCLC, 6 SCLC) were followed with daily CBCT during RT. The gross tumour volume for lung tumour (GTV-t) was visible in all daily CBCT scans and was delineated at the beginning, at the tenth and the 20th fraction, and at the end of treatment. According to discoveries selenium is a part of an active group of glutathione peroxidase enzyme. It prevents necrotic changes in liver, muscle dystrophy and has impact on growth. Extensive studies on content of selenium in plant and animal origin products were started in the beginning of the seventies in Western Europe (Frankenberger, et al., 1998). New York City data indicate that seasonal and annual variations in dispersion-normalized air pollutant concentrations appear to accurately reflect changes in source emission patterns. The normalized concentrations make it possible to observe the impact of regulatory changes on ambient air quality without these impacts being obscured by meteorological fluctuations. It is found that numerical modeling techniques and regression analysis can be powerful tools for extracting information from large sets of air quality data. The use of differential, as opposed to absolute, pollutant concentration values will reduce artifact correlations caused by seasonal, weekly, or daily meteorological fluctuations and will permit more accurate estimation of the regression coefficients. Analysis of Sr-90 in milk and tap water has been conducted in New York City since 1954. The milk sample analyzed is a monthly composite of pasteurized milk purchased daily at retail stores. The monthly Sr-90 to calcium ratios for New York City since the inception of the sampling progam in 1954 through 1981 are tabulated. Samples of New York City tap water are taken daily so that by the end of the month, approximately 100 liters have been collected. Data on Sr-90 since the inception of the program are presented. The available Cs-137 data expressed as the Cs-137 to Sr-90 ratio are given. A graphical presentation of the New York City Sr-90 data is also shown.

Even if there have been efforts to validate remote-sensing-based energy balance models, which provide evapotranspiration (ET) maps over a large spatial scale, few studies have been conducted to validate their performance in Korea. In this study, surface energy balance fluxes including ET were estimated and inter-compared with two different models using the Landsat-5 Thematic Mapper (TM) image over a watershed scale in Korea. While there were larger spatial discrepancies for net radiation, soil heat and sensible heat fluxes on a pixel-by-pixel basis between the two models, relatively smaller spatial discrepancies for the latent heat flux and daily ET were detected. The purpose of the present study was to examine whether a swimming exercise program would alter sarcolemmal enzyme activity, ion flux, and composition in rat hearts. The Interim Examination and Maintenance (IEM) Cell is located within the Fast Flux Test Facility (FFTF) Reactor Containment Building. This cell is a complex vertical hot cell whose purpose is to process reactor experiments and to perform maintenance on reactor and refueling components. Because access to this very complex cell is limited, a mock-up called the IEM Training Facility (IEMTF) has been developed. The IEMTF provides the IEM cell with many valuable benefits. Crop water demand in the Okanagan Basin was determined for 1961 to 1990, 2010 to 2039, 2040 to 2069, and 2070 to 2099. Daily station temperature data were spatially interpolated to a 1 x 1 km grid and adjusted for elevation. Daily precipitation data were estimated across four climatic regions. Output from three global climate models (GCM), CGCM2, CSIROMk2 and HadCM3 was used to create future daily climate. Rice, the world's major staple crop, is a poor source of essential micronutrients, including folates (vitamin B9). We report folate biofortification of rice seeds achieved by overexpressing two Arabidopsis thaliana genes of the pterin and para-aminobenzoate branches of the folate biosynthetic pathway from a single locus. We obtained a maximal enhancement as high as 100 times above wild type, with 100 g of polished raw grains containing up to four times the adult daily folate requirement.

Stroke-prone spontaneously hypertensive rats (SHRSP) induce spontaneous osteoporosis. To elucidate the specific characteristics of bone metabolism, the SHRSP was compared with age matched Wistar-Kyoto (WKY) rats. We investigated the effects of prolonged swimming exercise training on bone mineral density (BMD) and metabolism in the SHRSP. Seven-week-old male SHRSP and WKY were divided into three groups; the sedentary control WKY group (n = 6, WKY), the sedentary control SHRSP group (n = 6, SP) and the swimming exercise training SHRSP group (n = 6, SWIM) (in pool with 60 min./day, 5 days/week for 12 weeks). The OTS is a computer-driven system that provides real time response of essential FFTF plant functions to a control room mockup. The FFTF, a 400 Megawatt, three-loop, sodium-cooled fast test reactor will test fuels, materials and equipment for the U.S. Liquid Metal Fast Breeder Reactor Program. Construction is expected to be completed in August 1978. Initial criticality is expected in early 1979. This schedule will require FFTF control room operators to be fully qualified to operate the facility by late 1979. Because FFTF is like no other U.S. nuclear reactor, existing U.S. utility plants could not be depended on to provide highly experienced people to operate FFTF. We examined whether repeated viewing of a reminder video on a mobile phone would be an effective means of maintaining CPR and AED skills in lay responders. METHODS: : In a single-blind case-control study, 75 male students received training in CPR and AED use. They were allocated either to the control or to the video-reminded group, who received a memory card containing a video clip about CPR and AED use for their mobile phone, which they were repeatedly encouraged to watch by SMS text message. Specific objectives were to: (1) develop and refine a methodology that can be used to evaluate the feasibility of MAG-LEV trains; (2) apply this methodology to the state of Oklahoma. The methodology is based on an aggregate econometric demand model and mathematical programming. A city-pair network is constructed to evaluate alternative MAG-LEV routes between Oklahoma City and nine other cites in and out of the state of Oklahoma. Results obtained from the aggregate econometric demand model indicates that MAG-LEV trains would attract significant ridership along different types of origin-destination routes. All nine corridors exhibit induced demand for MAG-LEV trains as well as attracting riderships from other modes. Supplementing the collection of artificial neural network methodologies devised for monitoring energy producing installations, a general regression artificial neural network is proposed for the identification of the two-phase flow that occurs in the coolant channels of boiling water reactors. The utilization of a limited number of image features derived from radiography images affords the proposed approach with efficiency and non-invasiveness. Additionally, the application of counter-clustering to the input patterns prior to training accomplishes an 80% reduction in network size as well as in training and test time. Cross-validation tests confirm accurate on-line flow regime identification.
