The health politics and policy communities are still struggling with the question of "what went wrong" in the 1993-94 health care reform effort. Here I identify which Americans were politically active and inactive during the health care reform debate to explore the role political participation may have had in determining the outcome of the debate. Women's autonomy in household decision-making: a demographic study in Nepal

The aims of the study were to develop and test the psychometric properties of the Thai Family Health Routines (TFHR) scale, a 70-item self-report questionnaire used to measure the health of Thai families through their routine behaviors in daily life. Development of the TFHR was based on the structural domains of Denham's Family Health Model. The TFHR scale was initially composed of 85 items and tested on 1,040 families living in the central region of Thailand. Indications were that risk for mental health disorders, including Post-Traumatic Stress Disorder (PTSD), may be elevated in residents, and that this risk may continue for some time. New aspects for the evaluation of radioactive waste disposal methods

We analyzed the effects of the Colorado Medicaid Capitation Program on the duration and services of over 21,000 outpatient mental health episodes for young children. The study spanned a three year period before and after capitation was implemented, and compared episodes of outpatient care for children from 14 capitated Community Mental Health Centers (CMHCs) with those from three CMHCs that remained fee-for-service. Proportional hazards regressions revealed that capitation resulted in an increase in outpatient episode duration, over and above similar effects for the comparison fee-for-service episodes. The hypothesis that there were longer capitated episodes for children who were hospitalized was not supported. Children with behavioral and anxiety disorders had longer capitated episodes. For the performance assessment of radioactive and hazardous waste disposal sites, risk assessments are usually performed for the long term, i.e., over an interval in space and time for which one can predict movement and behavior of toxic agents in the environment. This approach is based on at least three implicit assumptions: One, that the engineering layout will take care of the immediate endangerment of potential receptors; two, that one has carefully evaluated just how far out in space and time the models can be extrapolated, and three, that one can evaluate potential health effects for very low exposures. Parents contacted?an average of four agencies (?1.7; range?=?1?14) in the previous year. Approximately one-half of parents were looking for either multiple types of treatment, or help for different problems, across agencies. The utilization of health care services in children and adolescents with chronic benign pain was studied in a Dutch population sample of 254 chronic pain sufferers aged 0-18 years. Children and adolescents who had reported chronic pain (continuous or recurrent pain >3 months) in our previous prevalence study were asked to keep a 3-week diary on their pain and to fill out questionnaires on background factors, health care use and the impact of pain. The balancedarmature- type loudspeakers, predominantly used in hearing aids, are inherently nonlinear devices, as any displacement of the loudspeaker diaphragm inevitably changes the magnetic and electrical characteristics of the loudspeaker. A numerical time-domain model capable of describing these nonlinearities is presented. Previous studies with adults with intellectual disabilities (IDs) report a prevalence of general anxiety disorder ranging from less than 2% to 17.4%. The Northwest Foster Care Alumni Study examined the relation between experiences in foster care and depression among young adults who spent at least a year in foster care as adolescents. Results indicate that preparation for leaving foster care, nurturing supports from the foster family, school stability, access to tutoring, access to therapeutic service and support, and a shorter and more stable placement history were associated with a lower probability of depression in young adulthood. A cohort of singleton black human females exposed to diagnostic x-ray in utero and controls matched by parity, hospital of birth and birthdate have been followed to ages 25 to 30 years in Baltimore, Maryland. The search for possible effects of prenatal irradiation has focused on health, growth, development, and reproductive experience of exposed and control women. This paper reports findings related to reproductive experience. From an original data set of 1458 matched exposed-control pairs of women, questionnaire responses were received from 1109 exposed and 1124 control women including 852 each from pairs in which both the exposed and control woman responded. There is a plethora of new silver-containing dressings on the market today. Various manufacturers attempt to show that their dressings are the most efficacious and therefore should be preferentially employed by health care workers based on the results of their in vitro tests. However, there have been no studies that clearly identify which tests are appropriate for comparison purposes. The purpose of this study was to determine which in vitro test is most appropriate for evaluating the antimicrobial efficacy of silver-containing dressings. This was done by testing seven silver-containing dressings and two non-silver-containing topical agents against 17 clinically relevant microorganisms using zone of inhibition assays and time-kill kinetic assays in complex media. Throughout much of the 1980s, deforestation in Brazil eliminated more than 15,000 square kilometers (9000 square miles) per year. That pace has only increased through the 90s and into the 21st century. Brazil is also home to more than a quarter of Earths tropical forests. Considering that the band of lush green that circles the globe through many equatorial nations is fundamental to the overall health of the whole planets environment, careful monitoring of forest health in the tropics is essential. Tropical forests act as major carbon sinks, places where ambient carbon dioxide in the atmosphere can be absorbed by growing things and sequestered for years. Definitive evidence shows that excess carbon dioxide can contribute to the greenhouse effect and speed global warming. Similarly, tropical forests also act as a primary producer of oxygen. The energy price volatility model suggests that some of the proposed natural gas programs can contribute to energy price stability. The sector most vulnerable to fuel price variations is, of course, the transportation sector. The most effective strategy to achieve energy pace stability is to reduce petroleum consumption in this sector. The natural gas vehicle program is therefore recommended as potentially important and worthy of further consideration. At this point, distinguishing the merits of various subprograms is not feasible. This result farther supports the conclusion that the DOE`s energy R&D portfolio is not efficiently balanced and an increase in oil and gas research should be a high priority. Body weight is a strong predictor of postural stability

A virtual balancing axis arrangement is described for multi-cylinder reciprocating piston machines for effectively balancing out imbalanced forces and minimizing residual imbalance moments acting on the crankshaft of such machines without requiring the use of additional parallel-arrayed balancing shafts or complex and expensive gear arrangements. The novel virtual balancing axis arrangement is capable of being designed into multi-cylinder reciprocating piston and crankshaft machines for substantially reducing vibrations induced during operation of such machines with only minimal number of additional component parts. Proper balance control is a key aspect of acitivities of daily living. The aim of this study was to determine the contribution of body weight to predict balance stability. The balance stability of 59 male subjects with BMI ranging from 17.4 to 63.8kg/m2 was assessed using a force platform. The subjects were tested with and without vision. A stepwise multiple regression analysis was used to determine the independent effect of body weight, age, body height and foot length on balance stability (i.e., mean speed of the center of foot pressure). With vision, the stepwise multiple regression revealed that body weight accounted for 52% of the variance of balance stability. The Savannah River Site (SRS) is a 310-square-mile United States Department of Energy nuclear facility located along the Savannah River (SRS) near Aiken, South Carolina. Nuclear weapons material production began in the early 1950s, utilizing five production reactors. In the early 1990s all SRS production reactor operations were terminated. However, the existing network null model, switch randomization, is unsuitable for metabolic networks, as it does not include physical constraints and generates unrealistic reactions. This case study, conducted from an interpretive paradigm, illuminates contextual factors related to the tutor experience when senior undergraduate dental hygiene students served as tutors for beginning undergraduate dental hygiene students, or sophomores, in a 1-semester, 2-hour long problem-based learning (PBL) course in a Baccalaureate Dental Hygiene (BDH) curriculum during the spring semester of 2008. Data were collected using various sources and methods. Six tutors and three administrators were interviewed, tutees completed an anonymous questionnaire, the tutorial process and tutor training sessions were observed, and related documents were examined. Data analysis included open and axial coding, creation of tutor profiles, and identification of patterns. During the second half of 1993, the groundwater at the F-Area Seepage Basins (FASB) was monitored in compliance with Module 3, Section C, of South Carolina Hazardous Waste Permit SC1-890-008-989, effective November 2, 1992. The monitoring well network is composed of 87 FSB wells screened in the three hydrostratigraphic units that make up the uppermost aquifer beneath the FASB. A detailed description of the uppermost aquifer is included in the Resource Conservation and Recovery Act Part B post-closure care permit application for the F-Area Hazardous Waste Management Facility submitted to the South Carolina Department of Health and Environmental Control (SCDHEC) in December 1990. Personal health information management (PHIM) refers to activities that support consumers' access, integration, organization, and use of their personal health information. We investigated PHIM in the health consumer population using a focus group and participatory design. In collaboration with health consumers, we identified PHIM activities and explored the design of new supportive technology. Our findings describe prominent PHIM activities such as monitoring and assessing health, as well as health-related decision making, planning, and action. We describe design principles our participants used during the participatory design of a PHIM tool. These include individual control, sharing, integration, security and flexibility. Investigations were conducted at Los Alamos National Laboratory to quantify the extent of migration of depleted uranium away from firing sites. Extensive sampling of air particles, soil, sediment, and water was conducted to establish the magnitude of uranium contamination throughout one watershed. The uranium source term was estimated, and mass balance calculations were performed to compare the percentage of migrated uranium with original expenditures. Mass balance calculations can be powerful in identification of the extent of waste migration and used as an aid in planning future waste investigations.

The present volume provides standardized energy balance sheets expressed in a common unit of tons of oil equivalent for all OECD Countries. It covers the years 1970 to 1982 year by year and includes many revisions and additions to data previously published. This work reviews the differences in fuel economy between two direct injection diesel engine versions through the use of a First Law energy balance and a Second Law availability balance. Both experimental data and simulated results are used in the analysis. The use of an engine simulation allowed the important processes in each engine to be analyzed and a Second Law effectiveness calculated. The availability balances and effectiveness values are used to determine the effect of major engine components on the fuel economy. A comparison is also made with an ideal engine with selected perfect or reversible processes.

Stream temperature is an important physical characteristic of headwater streams that plays a critical role in the presence and health of juvenile salmonids. Headwater stream temperature was documented in two geomorphic settings on the Kenai Peninsula, Alaska, focusing on the variation in temperature induced by diffuse groundwater discharge and variable air temperature. Eighteen headwater stream reaches were studied in four watersheds, with 11 drainageway sites and seven discharge-slope sites. In drainageway sites, low-gradient streams flow through broad valleys with groundwater-fed fen wetlands; in discharge-slope sites, high-gradient streams flow through narrow valleys with groundwater-fed slope wetlands. At all 18 sites, hourly stream temperatures were measured for one year. A new controlled-clearance pressure balance is under development with the aim of improving the hydraulic high-pressure standard up to 1 GPa. This pressure balance consists of three parts: (i) a pressure generation device up to 1 GPa, (ii) a weight-loading unit which can load/unload weights automatically and independently, (iii) a controlled-clearance piston-cylinder which is designed to allow the jacket pressure to be applied independently. Some adjustments were made for loading heavy weights on/off the piston safely, keeping them in balance, then generating the pressure stably. Stability of the generated pressure was checked for several piston-cylinders, and it was found that pressure fluctuation was less than a few parts per million. The jacket pressure coefficient of a 500 MPa controlled-clearance piston-cylinder was precisely evaluated as a function of both the system pressure and the jacket pressure.

The groundwater flow and transport model of the Faultless underground nuclear test conducted at the Central Nevada Test Area (CNTA) was accepted by the state regulator and the environmental remediation efforts at the site have progressed to the stages of model validation and long-term monitoring design. This report discusses the long-term monitoring strategy developed for CNTA. Subsurface monitoring is an expensive and time-consuming process, and the design approach should be based on a solid foundation. As such, a thorough literature review of monitoring network design is first presented. Monitoring well networks can be designed for a number of objectives including aquifer characterization, parameter estimation, compliance monitoring, detection monitoring, ambient monitoring, and research monitoring, to name a few. In this work the calculation of the average precipitation in the Quito River basin, state of Choco, Colombia, is presents through diverse techniques, among which are those suggested by Thiessen and those based on the isohyets analysis, in order to select the one appropriate to quantification of rainwater available to the basin. Also included is an estimation of the error with which the average precipitation in the zone studied is fraught when measured, by means of the methodology proposed by Gandin (1970) and Kagan (WMO, 1966), which at the same time allows to evaluate the representativeness of each one of the stations that make up the rain gauge network in the area. The economic benefits of supplemental fuel injections depend, in part, on the coke replacement ratio. An assessment of the accuracy with which blast furnace coke rate may be measured and a determination of the key drivers of coke rate uncertainty are offered, to provide guidance for experiments in high-rate gas injection. Using statistical analysis tools, an expression for the measurement error associated with the various terms of blast furnace carbon balance is developed. Coke rate calculations based on the material balance are most sensitive to coke carbon content and to proper tracking of hot metal tapping schedule.

When withdrawing a fluid sample (for additional chromatographic analyses) from an apparatus operated at a reduced pressure, a typical syringe proves to be ineffective (even if it is equipped with a gas tight plunger). It simply does not create enough pressure differential to remove a fluid sample from a reduced pressure environment. We encountered such a situation as part of efforts to extend the operation of the advanced distillation curve protocol to reduced pressures. The problem was solved by the development of a pressure balance syringe that allows reliable and precise sampling from an apparatus operating at sub-ambient pressures. This paper represents a parametric study of the amplification factor to account for rebound effects in the Energy Balance Method. Of the 66 distinct cases we chose for our parametric study, the amplification factor of 1.1 seems sufficient except in four borderline cases where the carbon steel pipes are small or have very small gaps between the pipes and the pipe whip restraints. We conclude that the amplification factor generally decreases as the parameters gap size, hinge-to-break distance and overhang increase.

A recursive construction of balanced realizations is associated with it, that possesses a very good numerical behavior. Several atlases of charts or families of local parametrizations are presented and for each atlas a chart selection strategy is proposed. The last one can be viewed as a nice mutual encoding property of lossless functions and turns out to be very efficient. Use of an Orientation Clinic to Reduce Failed New Patient Appointments in Primary Care

The primary responsibility of the Health, Safety, and Environment (HSE) Division at the Los Alamos National Laboratory is to provide comprehensive occupational health and safety programs, waste processing, and environment protection. These activities are designed to protect the worker, the public, and the environment. Many disciplines are required to meet the responsibilities, including radiation protection, industrial hygiene, safety, occupational medicine, environmental science, epidemiology, and waste management. New and challenging health and safety problems arise occasionally from the diverse research and development work of the Laboratory. Research programs in HSE Division often stem from these applied needs. These programs continue but are also extended, as needed to study specific problems for the Department of Energy and to help develop better occupational health and safety practices.
